Rectifier @ 14-04 was found to be smoking @ 3am. Flames were observed on the center coil when the lid was removed. Power was removed and bled. Water was applied to put out the fire.

The Oakdale portal elevator was inoperative from 8:55 PM until 11:10 PM on 3/31/2007. No injuries and no entrapements. Repairs were made by Schindler Elevator Service. An oil type overload device had leaked and had to be refilled.
An unintentional roof fall occurred on the 4th east headgate. The fall was approx 20' long, approx 18' wide and approx 10' thick. The fall did not affect ventilation or passage of workers. The fall will be cleaned up.
During a sig. winter weather event it appears snow and/or moisture entered the busway between the transformer and the switchhouse on a stationary 69kv to 4160v substation. A short-circuit resulted and the arcing caused a fire that caused damage to the substation. The fire burned for approximately 38 minutes after discovery and MSHA was notified at 37 minutes after discovery.
A roof fall occurred in the 1 west 3 mains, 67 crosscut, #3 entry. The fall measures approx. 20 feet long, 20 feet wide, and 5 feet thick. The area is supported with 5 foot fully grouted resin bolts.
A roof fall occurred in the 4th southwest submain #7 entry spad #29+16 at the 3-way intersection. Size of the roof fall is 18'W x 18'L x 6'T. Type of roof support is 5' fully grouted tension rebar bolts.
Discovered fall in bleeder entry off of 001-0 unit spad # 1117 - #4 entry. Fall app 20' long, 16' wide, 7' thick. No blockage of vent. No blockage of escapeways. No injuries. Fall will not be cleaned up - dangered off and breakered. Plotted on mine map.
Incoming electrical ""surge"" from CEI caused damage to 3 SCR's on the Production Hoist dynamic braking system. Mine was evacuated; no injuries or danger to personnel.
A roof fall occurred in the 2 west #3 mains, 85 crosscut, #5 entry. the fall measures approx. 12 feet long, 12 feet wide, and 5 feet thick. The area is supported with 5 foot fully grouted resin bolts.
HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 03/30/2007 at 08:55 am, the Cowling Portal Elevator was taken out of service to shorten the length of the hoist rope. The elevator was back in service on Monday, 03/30/2007 @ 01:30 pm. Each underground employee was informed to use secondary escapeways. A hoistman was stationed at the hoist house for the entire shift.
Roof fall: location MW return x-cut #22 approach to ME #3 seals. Dimensions: H n/a, w 19', l 40'. Geology: soft, medium to dark gray shale. Roof support-6'insal 3 bolts. Mined in 8/90. There were fall in area. Intersecton did not fall, did converge. This is the only access to the seals and has been considered unsafe for passage.
An unplanned roof fall occurred in the #4 face in the 4 right crosscut of the #2 section. Fall measured 20' x 20' x 7' high. Fall was on top of a Fletcher RRII roof bolting machine. No miners were affected by the fall.
A fall occured in 1D submain, #5 entry, at spad 22+30. The fall is 18' long, 18' wide and 8' high.
Unplanned roof fall occurred on #5 Belt 1 break inby spad #12186. No injuries. No equip. involved. Ventilation not affected. Passage of persons not affected. Fall measured 6' high & extended approx. 1 break long to belt line. All entrances to fall have been breakered off. Bolted with 48"" resin. Area of fall will not be rehabilitated. MSHA & State were notified immediately.
Roof fall above bolt anchorage, intersection of #4 entry B-53 North Submain. No persons involved. Area dangered off. Propsetter posts put in adjacent crosssuts and entryway to support brow area of fall and block access to fall area.
A fall occurred in the 1D submain, #3 Entry, spad 24+40. The fall is 18' long, 18' wide and 8' high.

A roof fall was discovered on top of the overcast. The fall was approximately 4' thick x 12' wide x 18' long. The fall damaged the overcast and was allowing return air from 003 section to mix with intake air going to 001 section. The fall was 1 x-cut over from spad #382. Both sections were idled immediately upon discovering this accident.

A roof fall occurred in the return of old workings. No equipment was in the area of the fall.
A piece of rock fell in the 2 north #5 mains, 64 crosscut, #3 entry. The rock measures, approx. 7 feet long, 4 feet wide, and 2 feet thick. The area is suported with 5 foot fully grouted resin bolts.
Unplanned roof fall above Anchorage Zone in 1st North Butt Room #12 outby Spad #2514 and inby Spad #2508. Two cross cuts in length 4'-4 1/2' high. Anchorage Zone was 36"" fully grouted resin bolts.
A roof fall was found in the #1 entry on the A3 panel at #40 block spad #2373. This area is approx. 5000 feet outby the face. The fall was 80' long x 6' high x 20' wide and was supported with 4 foot resin bolts. This area was mined in April of 2006. There was no injuries or equipment involved. It was found by the shift foreman.
A roof fall was discovered on this date. The fall was located in the #2 entry of the 7-right panel off 3-North #2 in the #5 crosscut. The fall measured 20' x 20' by 5'-6' in thickness. The area was orginally supported with 5' high tech bolts and 5' torque tension bolts.
There has been a roof fall outby spade #5539. 10-12"" thick and apprx 50'long. The area has been dangered off. It is not abstucting air flow and poses no danger at this time. A clean up plan has been posted at the site off the fall and a copy has been submitted to both state and federal inspectors.
A roof fall occurred at 1st North Main East, Crosscut 59, Entry 7. The fall measured approximately 25' long x 25' wide x greater than 9' thick. The fall did not result in any damage to equipment or property.

At approximately 1807 a miner was in process of installing ground support when approximately 15-20 ton of unconsolidated material ran from the unsupported back.
Roof Fall is in #11 x cut on #4 belt it is about 6' to 7' high 20' wide and 20' long.
West Mains 1, 7 block, 8 entry, 4-way, 20' long x 20' wide x 6' high. Support: 6' 3/4"" resin bolts, pans,cable truss bolts Fall dangeged off, additional roof support installed around fall clean-up plan developed and posted at fall site.
The miracle run elevator doors failed to open when the elevator returned to the surface landing. The shindler repairman determined that a door guide wheel pin had broken. The repairs were made and elevator returned to service at 1:55 am.
A non-injury roof fall occured on 10 west lw tailgate between the can crib and the last shield the fall measured 5' high, 5' wide and for a distance of 5 feet the tailgate travel way was blocked and blockage plan inplemented.
An unintentional roof fall occurred 700' outby the section in the No. 2 entry one break to the left of spad No. 5319. The fall measured 110' long, 19' wide and 0' to 8' thick. The fall does not affect the air flow or the travel of men.
A reportable roof fall was found in E-East Mains, entry 3, crosscut 12. The fall was approximately 25 feet long, 30 feet wide, and 7 to 10 feet high.
Overburden pressures caused the roof to deteriorate around the can and wood cribs impeding passage through the LW tailgate.
Main Portal Elevator was out of service until water was pumped from boot wash area where 2"" water line hose had blown. Lower landing emergency stop switch was replaced and elevator was put back in service at 9:45 a.m. on 3/26/07.
An unplanned ignition occurred in the East coal mill dust collector. The automatic Co2 system discharged and fire was extingushed in less than 30 minutes. MSHA was notified timely and later conducted a comprehensive investigation of the event. We are still waiting on the results of that investigation.
N6W face up/slip of rock. Broke from high wall. Fall was 20' in Width, 15' in height & 8' in depth.
An unintentional roof fall occurred outby the 002 working section in the 2D intake at station 75+90 entry #3. The fall measured 30 feet long x 25 feet wide x 15 feet high. The fall did not affect passage of men or ventilation.
An unplanned roof fall occurred on the #6A Belt 1 break outby spad #4223. Approx. 420' from the working face. Fall measured 80'x20'x10'. No injuries. Equipment was involved. Ventilation not affected. Passage of persons was affected. Bolted with 48"" glue bolts. MSHA & State were notified immediately. Area has been rehabilitated.
Occurrence at hoist house. Hoist was not running at time of occurrence. A forklift operator for another contractor went under hoist cable with forklift. When backing up, he hooked the hoist rope with the forklift, pulled cable back causing damage to hoist house. The rope was kinked and had to be replaced with new rope. Job site down from 3:30 pm 3/24/07 until 1:30 am 3/26/07.
A roof fall occurred in #5 entry #4 crosscut of the 2nd Mains West. The fall was 20' long, 20' wide and 8' high. The area was bolted with 6' & 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.
On March 24, 2007, on the west mains section, the continuous miner,while cutting sandstone and shale top, generated frictional heat allowing a piece of cap coal to start burning in the no. 9 entry. It was extinguished in less than five minutes. The miner cutting coal was an Eimco 2810-0. There was no pop, no charring, no visible signs of ignition, only fire.
Inspection of # 7 Hoist, showed a low voltage indication. Electrical inspection concluded that a lightning strike had taken out a potential transformer that was tied into the MG power to the # 7 hoist. 3 of our other hoists were operational.
Mine lost power. Employee was send sent to investigate and found a roof fall in the main line just inby the Rilda intake portal. The roof fall was above the anchorage zone and blocked passage of personnel. Mine called MSHA and inspector came and investigated the roof fall.
7 Mains 2, 5 Entry, 2 Block, 4-Way, MMU 001 20 ft long x 17 ft wide x 6 ft high. Support: 6ft 3/4 inch resin bolts, pans, cribs. Fall dangered off, additional roof support installed around fall.
An unintentional roof fall occurred at the tailgate of 8I section at 77+80. The fall measured approx 16' long X 16' wide X 8'-10' high. No injuries and no equipment damaged.
Fire in the Lime Kiln Preheater. A contractor was working in the preheater installing refractory. They were using wooden scaffolding and wooden forms which were ignited by an undetermined source. The local fire department responded, but did not enter the preheater, the fire burnt itself out. The fire damaged several steel beams, but no major structural beams were damaged.
While removing a ramp in the EOB-A drive a portion of the right rib failed and began to break out under the supported ribs above. These were pulled out past point anchor on 3/23/07. On 3/23/07, we determined that some boltz fellout of the rib above. The right rib went in approx 15' h x 15'w x 10-15' into the rib.
A fall of roof occurred in the 3A Submains @ cross cut # 2 in the # 4 Entry. The fall was approx 30' long x 18' wide x 10' high. The fall obstructed ventilation.
Fall of roof discovered by belt examiner on 3-23-2007 at 2:50 pm. Fall was located at the mouth of bleeder panel Southwest Mains #2 heading in the returns entries. The fall measured approximately 0-10 feet thick x 19 feet wide x 40 feet long. No equipment was involved or the ventilation was not interupted. The area was bolted with 5 foot torque tension bolts.
A roof fall 20' wide X 180' long X undetermined height was found in 9 main rooms #4 Butt old working 2 cross cuts inby station 4159. The fall does not impede travel or impair ventilation and was dangered off.
At approx., 1:45am, the loader operator was notified by rock truck driver that the loader was on fire at the rear of the cab. Both EE's used their fire extinguishers to put out the fire. However, the fire started burning again. This time a call was made to 911 & MSHA was notified @2:16am when it was realized fire had burned for 30 mins. without being extinguished.
An unintentional roof fall occurred on Unit 1 MMU (001) working section in the last open crosscut between #3 and #4 entries at the +1885 foot mark. The fall occurred due to the failure at just above the anchor point, slip broke out from the roof at approx. 5-6'. The fall measured approx. 18' w x 40'. 1x5-6'. h. The roof was orginally supported with 5' conventional type roof bolts.
An unintentional roof fall occurred in the #4 entry at the #67 crosscut on the #3 belt. The fall measured approximately 20' wide x 6' high x 40' long. Area was supported by 4' fully grouted bolts. No injuries, no equipment caught & ventilation was not interrupted.
The elevator in #6 airshaft was inoperable due to the brake being set at 94%. The brake was re-set and the elevator operated. One person was on board. The elevator was only down for 23 minutes with one person on board. We do not feel this is a reportable incident under Part 50.
Hydraulic piping problem on slope hoist when a fitting came apart. Replaced broken fitting and refilled with oil. Hoist was put back in service at 3:59 p.m.
A trussbolter that had been used in a rehabilitation area in the no. 9 seam caught fire. Carbon Monoxide from the fire was detected by the Atmospheric monitoring system sounding an alert. The alert was responded to and the fire discovered. The mine was evacuated and personnel were sent to fight the fire. The fire was extinguished at 10:03 PM.
A roof fall occured on 3a submain, #1 entry crosscut 34. The fall is 40' long, 18' wide, unknown hight.
The page portal service shaft elevator was taken out of service at 10:05 am on 3/21/07 in order to change the main hydraulic pump. The pump was changed and the elevator was put back in service after test runs at 11:15am om 3/21/07. There were no people on theelevator and no injuries.
Brakes locked up on brake car when hoisting up the slope from bottom switch. Corrected hydraulic leak, filled with oil and put back in service at 9:00 p.m.
The directional solenoid valve malfunctioned and the slope car would not travel to the surface. The solenoid was replaced and the slope car was put back into operation.
At approximately 2:15 am the tail gate travelway of the longwall was observed to be impassable by roof screens that were filled with rock from the immediate roof and some of the rock from the immediate roof had fallen on the mine floor. The area involved was approximately 60 feet long. This is a tail gate closure.
An unintentional roof fall occurred in the intersection of the South East Mains in the # 8 Entry at 33+05. The cause of the roof fall was laminated and unconsolidated shale at the anchorage point. The area had been supported with 6' fully grouted roof bolts.
A roof fall was found by MSHA Inspector and mine examiner while examining the A-3 Mains, 1st Parallel Panel, 3 Blocks outby Spad # 2450. The roof was approx 10' high x 70' long x 20' wide. The area was supported with 4' resin bolts. This area was 7000 ft outby the face.
Roof fall occurred on Headgate 18 #4 and #5 entry. The fall measured approx 20' wide, 6' thick and approx 3 brks long. Section was developed approx January of 2006. Supported with 6' torque tension bolts. Area has been dangered off and timbers and cribs to breaker off fall.
A fall was discovered in the intake cushing at a stopping, which will be rebuilt. No injuries and the fall will not be cleaned up.

While mining through a barrier block water was cut into on the cut through. No injuries occurred.
The slope car serving the #1 track slope was inoperable due to over-ride from 8.42 p.m. until 9.25 pm a fuse was replaced and the slope car became operable.
A roof fall above the anchorage zone occurred on the B Mains Left haulage at the intersection of 14 room. All approaches are dangered off and the intake escapeway has been re-routed. All agencies have been notified accroding to the law.
An unplanned roof fall occurred on the #7 belt at #9 entry and spad no. 7264 outby one break. The length of fall 60', width 18', height 4.5' - 5.5'.
#5 Entry fell in at #5 x-cut in Return. It is about 5 1/2' to 6' high, 20' long & in Entry section 300' inside of mine. Water is coming through the top & it had a hill seam in several places.
Ignition occurred on the Headgate Drum of the Longwall Shear while coal and mine roof was being cut. The Shear was cutting coal and rock while coming toward the Headgate. The ignition occurred at No. 53 Shield.
On Monday, March 19,2007-Leadman called the surface office about 8am to inform me of a roof fall at the intersection of NE 10 @ U. This area had previously been closed off to access on 2-6-07. This area was not an active mining area. The roof fall was approximately 30' wide and 40' long forming a bowl in the roof roughly 20 feet high. MSHA was immediately called.
The bottom landing door on the Livingston elevator was damaged by ice. The elevator itself was not damaged.
On 3/19/07, we discovered that someone had broken into the quarry, and cut the fill hose on a 20,000 gal. diesel fuel tank. About 4,000 gallons of fuel spilled onto the ground, outside the secondary containment. KY DEP, NRC, MSHA, and local police/fire departments were all notified. TPM Group was contacted to help coordinate the cleanup efforts.
Roof fall on #3 section. Fall occured in #2 entry at spad #522 and extended through cross cuts over to # 5 entry at spad # 507 for a distance of 250 ft adn 15-18 feet thick
After taking the second lift (#42 on the pillar cut plan) a piece of draw rock fell cutting the #2 MRS cable. This delay allowed the roof to work, then fall on the #1 and #2 mobile roof supports before the cable could be repaired. The fall measured 18' by 28' long and was 4' to 5' in thickness.
The continuous miner was cutting in number 1 face when a small instantaneous flash occurred.

The hoist operator detected a problem with the production hoist. After troubleshooting it was determined that a contactor had stuck ""open"". Mine was evacuated. No hazards/injuries. Hoist back in service at 6:55pm

The Oakdale Portal Elevator was shut down and inoperable from 7:50 am to 10:30 am. The intentional shut down was to perform repair work. No entrapment, no injuries. Schindlers elevator service repaired a door closer cable. Elevator was returned to service at 10:30 am.

A mine examiner found a roof fall in the 1st Right Section, #29 xc in the old belt entry (#2)
An outside supply crew was pulling a longwall shearing machine onto the man/supply hoist when the move car came uncoupled. The draw bar of the move car damaged 2 side panels and a speed gauge housing box was knocked out of position.
A roof fall occurred in the 2 North #5 Mains, 65 crosscut, #3 entry. The fall measures approximately 25 feet long, 18 feet wide, and 14 feet thick. The area is supported with 5 foot fully grouted resin bolts.
Elevator was put on hold by the operator as a precaution till it could be checked by the elevator service. The elevator was not out of service because of failure.
Roof fall found in the beltline of the worked-out E-8 section. Area had been supported using 4-foot fully grouted resin bolts or 6-foot Install II bolts (6 bolts per row) using 6 x 16-inch plates. Immediate roof consisted of shale with sandstone laminations at 8 to 10 feet. Mine ventilation was not affected.
The Cedar Creek elevator was out of service from 9:50 pm 3/16/07 until 12:15 am 3/17/07 due to a door malfunction.
An unplanned roof fall occurred in the 2 Right E Butt section, in the #3 entry (belt entry). The fall was approximately 90 feet long and 4' plus high, between Station Spads #922 and #927. The fall did not affect ventilation. MSHA officials were notified as soon as the fall was discovered, and the place was timbered and dangered off.
Employee became stuck in elevator.
A 12'x 12' rock rolled down the mountain and came in contact with the fan and knocked the fan to the side and out of the entry.
A roof fall measuring 20' L x 20' W x 7'H occurred in Panel 4 off of panel 1 submains. 2500' outby active 005 MMU (003 section) in number 3 entry at spad # 8870. Area was bolted with 6' TT bolts.
The page portal service shaft elevator went down at approx. 6:05 pm on 3/15/07 due to a blown 0-ring ont he brake release valve. There were no people ont he elevator at time of incident. The o-ring was replaced and the elevator was tested and put back in service at 7:05 pm.
The slow down switch for the #2 hoist failed to function properly. The mine was evacualted and the slow down switch was replaced with a new one. The hoist was tested and miners returned underground at 3:57 p.m.

A roof fall was discovered on 4 north mains in #2 heading at brk. 89 to 91, inby spad #18288. This fall does not impeade ventilation, no injuries, and no equipment was involved. The fall was timbered and dangered off at all approaches nad it will not be cleaned up.
A fall was discovered measuring 60'x30'x6' by the mine foreman and state inspector at ss7890. There were no injuries and it will not be cleaned up.
An unintentional roof fall occurred in 5I bleeder entry #3 - 5 wall. The fall measured approx 60' L x 15.5' W x 10' - 12' H. No injuries & not damage to equipment.
A roof fall was discovered in the 2 north #5 Mains, 51 Crosscut, #6 Entry. The fall measures approximately 35 feet long, 18 feet wide and 7 feet thick. The area is supported with 5 foot fully grouted resin bolts.

Discovered that the roof had come in adjacent to the last shield on the face and travel off the face in to the tailgate entry was blocked. The face was located at survey station 117+60 at the time of the fall. The fall was approximately 18 ft wide, 7 feet high and 15 ft long.
A fall of roof occurred in the 3B Submains in the #4 Entry at x-cut 15. The fall was approx 30' long x 18' wide x 8' high. The cause was determined to be an undetected slip with water.
During the weekly examination, a reportable roof fall was found in the Sly Gulch Return, entry 1 - 2, crosscut 16. The fall was approximately 16 feet long, 8 feet wide and 5.5 feet high.
Our weekly examiners found a roof fall in the 3 Left - 4 West panel in entry #2 between crosscut 27 and 28. The fall was located in an entry used by the examiners on their weekly air course inspection.
The cedar creek elevator was out of service from 2:00 am until 4:45 am on 3/13/2007 due to door rollers off track.
A fall was discovered measuring 20' x 20' x 8'. No injures occurred as a result. The fall will not be cleaned.

A roof fall was found by our weekly examiners on the 3 left - 4 west panel in entry #2 at crosscut 26 to 27. The fall was located in entry adjacent to the entry they normally travel while making their weekly examinations.
An unplanned roof fall occurred on the #1 section in the #4 entry at spad #17570. No injuries. No equipment involved. Ventilation & passage of persons not affected. Fall measured 50'x20'x10'. All entrances to fall have been breakered off. Area of fall will not be rehabilitated. MSHA & State were notified immediately. Federal inspector investigated the fall on 3/13/07.
Unintentional roof fall discovered at 7:40AM. Fall measured approximately 20'L X 20'W X 10'H. Intersection was previously supported with 6' full grout resin bolts and 10' long cable bolts. Area was mined the week of 03/15/2007. At the time of this report, the 4 approaches to the fall will be cribbed off.
An air valve on the service hoist brakes was sticking that caused the hoist to kick out in automatic. During troubleshooting the hoist would not go back into service immediately, so the mine was evacuated. MSHA national reporting hotline was notified when the evacuation was called at 11:36 a.m. and after the hoist was back in service at approximately 2:05 p.m.
An unintentional roof fall occurred on the 3rd East Longwall Tailgate Entry. The fall was approximately 20 feet long, approximately 18 feet wide and approximately 8 feet thick. The fall did not affect ventilation. The fall will be cleaned up.

North 4, 63 Block, 5 Entry, 4-Way Intersection MMU 004. 10 ft long x 10 ft wide x 6 ft high Support: 6 ft 3/4 inch resin bolts, 12 ft cable bolts, pans, brow straps, screen, jacks. Fall dangered off, additional roof support installed around fall. Clean up plan developed and posted at fall site.
Roof Fall 2 West belt neutral, #3 entry, x-c24, 30'x30'x10', the fall has been dangered and barricaded off, will not be reclaimed, section has moved to rooms to the right side of 2 west.
The crew noticed that their belts went down. Upon investigation they saw that the roof was working and left the area. When they came back they found an intersection containing the lo-belt drive had caved. The area was bolted with 42 inch fully grouted resin bolts. The fall is approximately 20 ft x 60 ft x 4-6 ft high. The area has been cribbed and dangered off.
Ground fall occurred at 3750-AX200 heading, the fall occurred in the back of a drift that was being prepped to be jammed. A front end loader was cleaning the area when he noticed the ground starting to dribble. He pulled out of the area and called for the lead miner.
Fuse on ""B"" phase of thyrister drive for 4703 hoist failed; no indication that failure was an overcurrent condition.
Roof Fall found on preshift exam, End of 2 West #7 entry, x-c34, fall was 40'x30'x10-12'high, mining had already ceased and had moved to rooms on the right of 2 West, fall was dangered and barricaded off, no clean up to be done.
An unintentional roof fall occurred in the 8th North Headgate unit, Entry #1, Crosscut 42 at 1611 North. The fall was approximately 46' length, approximately 36' width and approximately 10-12' thick. The fall will not be cleaned up.
An unintentional roof fall occurred in the track entry at Break #66. The fall measured approx 41/2'-5' high, 20' long & 20' wide.
Non-injury accident. Employee was using tourches to cut bolts from old fuel tank after he was told not to. Hot slag fell into old fuel dike starting fire.
While removing a brick from the secondary quench tower, a brick fell from the hoisting bucket landing on another brick at the bottom causing a spark which resulted in a flash of sulfur dust and ignited residual sulfur in the bottom. The employee exited the quench tower with the assistance of the confined space attendant; fire was extinguished within approximately 15 minutes.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to the structure at the bottom landing rubbing the elevator. The structure was repaired and the elevator placed back into service @ 2:45 am on 10/10/07. The alternate escapeway for #1 end was re-routed to the slope. all necessary government officials were contacted and notified according to law.
Upon an investigation the dozer operator was pushing/spreading coal to permit an additional storage radius around #2 stacker. During pushing/spreading the coal an undetected bridged area broke free creating a void at the #4 fixed feeder, the dozer proceeded to slid down into the void backwards and rested in a vertical position.
A fire was found in the #10 Belt Drive 300 KVA Belt Power center located 2 Breaks outby #10 Belt Drive. Fire was contained in the transformer and was extinguished in 15 minutes. No injuries resulted from this incident.
A roof fall measuring 10' high x 20' long x 16' wide occured ont he 10 left 1 south development section at no. 35 block. the fall occured in a crosscut between the stopping and the no. 3 entry. the area has been cribbed off on both sides to secure the top from furture activity and dangered off to prevent persons from entering the areas.
The elevator in the #6 shaft became inoperable when the safetronics card malfunctioned with twenty persons on board. The elevator was down from 4:35 p.m. until 6:55 p.m.
A fall of the Roof occured on #2 unit, in the 2nd South East SubMains, #7 Entry, last open crosscut. The fall measured 20 ft(length) 25 ft(Width) 6 ft(Thick). Personell, ventilation or equipment was not affected.

A roof fall occurred in the 4 South Panel #9 entry in the pillarline between crosscut #13 and crosscut #14 measuring approximately 35ft. x 19ft. x 5ft. The area was supported by 5 foot fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
Outburst - 1400' level - room 29 B-east. Outburst occurred during a shot. No one was in the mine at the time. Miners returned to production work in the mine after it ventilated for approximately five days.
The Miracle Run elevator doors would not open. The Schindler repairman determined that a steel cable that opens & closes the automatic doors was broken. The cable was replaced & the elevator was returned to service @ 1:30 pm.
A PRESSURE REGULATOR VALVE FAILED, ALLOWING 35 PSI NATURAL GAS TO GET TO THE BURNER, RESULTING IN AN EXPLOSION WHEN THE DRYER WAS SHUT DOWN.

On 3/6/07, an unintentional roof fall occurred on the 2nd W. sub-main off the 3rd south in #8 entry at cross cut #23. this roof fall was supported with 6 and 10' point anchor roof bolts the roof fall measured 20' long x 18' wide x 10' high. the roof fall will not be cleaned up.
Anunintentional roof fall was found in the return entry of the G6 section between x-cut 15 & 16. the fall approx 12' wide x 60' long was just above the 4' roof bolt anchorage.
A roof fall was found by our weekly examiners in the 3 Left - 4 West panel in entry #7 at crosscut 36. The fall was located in the entry adjacent to the primary escapeway for MMU-005.

Ventilation was affected. Passage of persons was not affected. All entries to fall have been breakered off. No equipment involved.
A roof fall occurred in the 1A submain, crosscut #12 in the #1 entry. The fall is 8 to 10 feet high, 22 feet long and 18 feet wide.

Water in the immediate roof caused deterioration of the roof. This caused the roof to fall above the anchor point. The fall was located on the 0 West cm section at spad number 41 + 61. The fall occured 2 cross cuts inby the section loading points. Fall was 17' long, 17' wide and 9' high. Area was mining in March, 2007. Fall was in #2 entry.
There was a roof fall measuring 20' W x 40' L x 5' H at the intersection of break 50 on 2 south section. This was reported to MSHA in the required reporting time period.
An untintntional roof fall was discvered at approx. 6:30 am at brk 67 on 4 north mains in #2 heading. There was no injuries, no equipment involved and ventilation was not impaired. This fall will not be cleaned up. The fall area was timbered and dangered off at all entrances.
A roof fall was found at x-cut 92 Main south between entry 2 and 3 on March 3rd. Area had been mined August of 1991 and was bolted with 4' foot resin pins. No injury or property damage resulted.
Control fuses blew out during a power outage and no replacement fuses were in stock. The hoist car serving the track slope was unavailable from 1:58 p.m. on 3/3/07 until 4:30 p.m. on 3/4/07.

The #1 elevator was in-operable for a period in excess of 30 minutes due to a bad buffer switch. The buffer switch was changed and the elevator placed back into service.
To the best of my knowledge, a hydraulic hose burst and caused a fire. It took 2 1/2 hours to put out. We were unaware we had to report to MSHA.
A ventilation interuption to the #1 fan occurred due to a power outage for a period in excess of 15 minutes. Power was removed from the rest of the mine and all employees were evacuated according to law. All neccessary agencies were notified. A special exam was performed after ventilation was restored.
The weekly emergency hoist was being load tested for the monthly test. After the capsule had been lowered to the bottom, it could not be brought back up. The emergency hoist was inoperative.
A roof fall occurred in the 1 South 4 Panel, #4 entry, 16 crosscut. The fall is approximately 35 feet long, 20 feet wide, and 7 feet thick. The area is supported with 4 foot fully grouted resin bolts.
Cause of accident is unknown at this time. Employee was working on a winch on a dredge boat and was found drowned behind the boat some time afterwards.
4-employees were performing preventative maintenance on the aerial tram system used to transport stone from the quarry to the plant. While tensioning the steel cable used to carry the tram buckets, the hydraulic cylinder assembly pulled loose from its anchor point. One employee was fatally injured and another suffered a broken leg, after being struck by components of the system.
While excavating coal pit, equipment operator worked himself into a position that when he attempted to correct resulted in overturning excavator on its top, crushing the cab area and the operator.
Smashed right hand middle finger tip area in tailgate (fracturing) on dump truck.
EE had his left hand on the drill pot as it was being raised. His left little finger got caught between the leveling arm and feed jack cylinder.
Employee stuck hand in head pulley conveyor.
EE was greasing a tail pulley while it was running.The guard was removed by him to clean grease zerk, glove was caught by pulley & pinched his ring finger on the right hand.
Readjusting plant level, hand slipped causing him to fall forward and his middle finger went into a hole, resulting in him loosing approx 3/8"" from his middle finger.
EE was operating the Thin Veneer saw; a rock got stuck in the saw blade; he was pushing it out of the way when his finger got caught between the rock and the saw blade; his left ring finger was cut off at the first knuckle.
Employee was operating a shuttle car with his left hand on top of the canopy when his hand was caught betweeen the mine roof and the canopy.
Individual was attempting to pull a matrip to motor barn with another mantrip. The mantrip being towed derailed in a curve and landed on his right foot resulting in fractures. Individual was communicating with operator of the other mantrip while standing beside the mantrip.
Employee was operating a forklift at the Packing House area. Suddenly he saw a defective cement bags pallet inside the palletizer. He entered inside the palletizer avoiding the safety devices used to stop the machine. The pallet continued its movement pinching accidentally his index finger between the pallet and a conveyor roller.
The employee was cleaning ""Tek-Seal"" material from the hopper of the Micon pump and his glove got caught in the auger.
The employee manually lifted a shop-made trailer to attach it to a truck. He dropped the trailer and it landed on his finger. The tip of his index finger was severed.
The employee stated he was setting timbers when he felt pain in his stomach. He did not report anything until 3/9/07. He was diagnosed with a hernia to his stomach on 3/21/07. He continues to work.
Employee reported some pain & discomfort in his left abdomen. We discussed it & we decided to rest his side after a weekend & see if the pain went away. The pain did subside. Then a couple of weeks later, he said the pain was back. He went to see the Dr. The Dr. put him on restricted.
Employee was helping the electrician put a lid on the power center. The lid dropped on employee's left hand index finger. It injured the area from the last knuckle up. The doctor later removed the tip of his finger from the last knuckle up.
Employee's thumb was caught between forklift cage and handrail as he was attempting to hook cable choker around forklift to remove welding machine from upper deck of rail loadout.
Pulling chain on longwall set up face. When employee got finger caught between cable and hand rail.
Individual came outside after shift and remembered he had to deliver a contractor back to the section for the miner. When returning to section he went to the miner and bent down to look into the panel. When the contractor went phase to phase causing an arc flash burning his face and hands.
Climbing two steps, sprained left knee.
Loading supplies onto bolter. Turned with plates, hit left hand on tram lever, injured left little finger. Taken to e.r. for treatment, stitches used.
Employee was operating roof bolter on #1 section, installing a 5' rib bolt with board when a piece of rib fell off hitting his head and left arm.
Slid Dozer off end wall to the pit floor about 10'. He was offered transportation to a hospital, he refused. Then in Dec. 07 started complaining of shoulder and neck pain, said was caused from acccident in 3/07. He started missing work and had surgery on neck and shoulder on 3/10/08. This was reported to me on 3/25/08.
The EE was using a wash down hose to clean a plugged chute when another EE increased the pressure on hose causing the EE to lose his balance striking his right arm on a steel support beam. The EE did not stop work however on 4/18/07 he was diagnosed with a torn rotator cuff & was scheduled for surgery. He started missing work on 4/18/2007.
Employee was building seals and got mortar inside boot.
Employee was waiting for crew to bring in a set of doors to install. He heard motor approaching. As he got up, he stepped on a piece of cinderblock and twisted his ankle.
Operator of the 17 Scraper was fixing pot holes when he hit a pothole, straining his back. He was taken to the ER, given a prescription and taken off work. Ongoing.
EE was installing a set of air lock doors. While walking over to check a brace he walked past a rubber tired four wheel personnel carrier that was sitting on a slight incline. While in front of the four wheeler, the operator released the brake which allowed the four wheeler to roll forward, striking ee on the right leg.
While employee was performing his normal duties as a Scoop Operator in low coal the employee experienced swelling in his left knee. Employee continued to work until 04/09/2007 at which time he started missing work making this a reportable.
Miner was helping change steel on drill when he accidentally got his hand to close to jaws that hold steel, smashing the end of his middle finger on right hand. Note: I had cautioned him 5-6 times about not getting too close to the moving parts, jaws, etc. (His last day of work was 4/9/07.)
Miner strained lower back lifting a transmission cover. Note: Did not begin missing work until 11/20/2007.
Employee thought a pallet was going to fall and grabbed pallet stubbing thumb.
While stacking pallet, slipped and rolled foot over, resulting in chip/fracture to foot.
Employee & co-worker moved crusher with chain & excavator. Employee fell. Crusher moved about 2"" & caught back of his heel.
Unit #1 - Employee was hanging curtain off a ladder, top was 10' tall. Employee fell from ladder and landed in pile of coal resulting in fractures above and below the left knee.
EE was lifting & loading rebar repetitively throughout the day.
EE loading header boards by hand onto a flat car to be transported underground when he set the boards down onto his left index finger. He was transported to the hospital and recieved 4 stitches to the tip of his finger. EE ret'd to duty next shift and continued normal duties.
He was standing by the loading machine when the cont miner was positioning to cut a corner. A cable hanger & miner cable & water hose fell & struck him on the back of the neck causing contusions to the neck & head.
He stepped out of a bulk truck, stepped on a rock in mud, turned ankle, hurt (broke) right ankle.
Employee stopped her truck and got out to move a ten inch rock at dump site. While she was getting back in her truck she said her back popped.
Installing bolts, bent bolt to put into hole, when straightening bolt, injured left shoulder.
This injury was not reported. Employee called in 4-2, said he had back pain. Employee called Wed. 4-4 and said he strained back the prior week.
Injured and co-workers were attempting to dislodge filter cake stuck to the inside and door openings of the stacker tube, when some loose material fell from the stacker. Injured was struck by the material, fell and rolled down the side of the 5 foot high coal pile that he was standing on. Injured had the air knocked out and felt pain in his lower back and right side.
Injured employee was a stand-by person on a high pressure water blasting task. The person controlling the water blaster tripped and fell backward. As he was falling, he accidentally hit the injured employee with the high pressure water, causing a puncture/laceration wound to the employee's right forearm and right side of chest.
Employee was exiting a 400 Komatsu by walking off front track. He stepped off track with one leg onto the ground. The weight of his body was not supported, causing his leg to bend backward and him to fall.
Employee was helping to load belt in the belt core when a co-worker's channel lock slipped, causing employee to have the full load of the belt, causing him to feel a strain on his back.
Employee stated that while climbing on the powder truck he fell/slipped and hurt his back. Reported to Management 4-3-07. Went to Doctor 4-6-07.
Dozer was being serviced with blade safety stands under push arms. Operator was climbing back up on dozer when he slipped & fell fracturing left wrist.
Injured was bolting top when piece of rock fell striking injured on head right shoulder.
Employee stated after hanging continuous miner cable to the mine roof he went back to operating a joy shuttle car when he started experiecing lower back pain. He was seen at a local area emergency room and was released to full duty. His back pain continue to worsen until he went back to the hospital to seek medical attention on 4/2/07.
Employee was dismounting that #1 dragline, descending the steps between the house staircase and the top of the shoe. He slipped and fell, striking his elbow and back.
Aggravation of previous left knee injury from 12/19/06 - was off work from 1/24/07. Returned to work on 3/12/07. Returned for treatment on 3/28 and off work since that date.
Employee slipped and fell landing on his left shoulder. He did not seek medical attention initially, but did eventually get treament and later had the injury repaired surgically.
Walking beltline he stepped across water line he stepped and straddled the watch line pulled muscle in his back.
Employee was cleaning around swingers on dragline with loose rag. Loose rag caught and was pulled through the guard. This action also pulled employee's hand up to guard.
Employee was stepping over a 10"" drisco pipe, right foot was over the pipe, foot gave way in sand. When employee tried to catch himself, his left foot stuck and he heard a pop in his left ankle and felt pain. He was taken to the ER, diagnosis pending MRI, taken off work. Currently Ongoing.
Employee was trying to get out of scoop when his right knee popped. The canopy was very low making entry and exit very difficult.
He lowered the drill box onto his foot.
While carrying a 6"" X 20' aqua pipe the EE stepped on a lump of coal and sprain his ankle.
I was hanging miner cable on rib; I picked up the cable and felt a pull in my neck.
The alleged injury was to have occurred when the EE picked up a miner cable to lay in the clear when he stepped in a hole in the bottom resulting in twisting of his right knee. EE cont'd to work until 4/24/07 at which time surgery was done to right knee.
Employee was carrying two stopping blocks to a stopping that he was building when he tripped over some block bands causing him to fall.
Employee helping water batteries and was replacing lid on batteries when another employee got on a 3-wheeler and the 3-wheeler rolled back pinning his left leg against the bumper of the 482 fork lift.
Mounting water truck using the grab bar and felt something pop in his left elbow. (Did not seek medical attention until later.) Missed 8 days from work.
Unit mechanic was pulling on miner #33, cutting loop cable strap to place on cuting head of machine when he felt a pull and burning pain in lower left leg.
Climbing ladder to enter cab of haul truck, employee felt a pop in his knee. He was originally diagnosed with a knee sprain. Further treatment and MRI revealed a torn meniscus. Surgery was performed on 8/28/2007
Employee was usning a utility knife to cut a rope-type cable hanger. The knife slipped and he cut his left thigh.
Employee was going to get a tool and tripped over ditch. He fell on his knee requiring stitches.
Attempting to remove wheel from rockduster. They had removed the axle pin from the wheel, jacked the duster up, placed wooden header boards under frame to provide clearance for the wheel to drop. Rubrail bar was bent & there was pressure on the wheel which caused the wheel not to break loose. EE struck wheel with hammer which caused the frame to drop & rubrail to catch top of foot.
EE was steam cleaning a loader using a Landa Pressure Washer. He climbed on the tire and slipped. He landed on his back.
EE was struck in the jaw by an auger flight while hooking a wrench to a bit.
EE was dismounting from a mack800 powder truck, put his foot on the ground, his foot slipped and EE twisted his ankle.
Placing trolley pole on wire, felt pain in abdomen. Diagnosed on 5/7/07 with an umbilical and bilateral hernia.
Crew were taking a belt drive apart. The boom was being held up with come-along while pins were removed. Chain broke letting boom fall down against drive frame catching employee's hand.
EE was welding an I Beam onto the highwall miner. As he was doing this, a piece of coal about the size of a football, fell from the highwall and struck EE in the back. He was transported by vehicle to St. Marys Hospital in Lafollette, TN. After x-rays, EE was diagnosed with a deep bruise to the back and no bones broken. EE did not start to lose time til 5/4/07.
Employee was assisting in the removal of a 3'x12' screw conveyor housing. The housing was placed on a 4'x4' wheeled dolley. As the housing was removed from the work area it rolled off of the dolly and struck the employee's right foot.
The roof bolter operator was preparing to drill a hole to install a roof bolt. As he started to rotate his drill steel his glove got caught on the drill steel causing his left hand to be wrapped around the steel resulting in a laceration to the little finger and ring finger.
While loading roof straps onto the miner, a bottom strap became fouled and the upper strap that he was pushing on continued to slide over the fouled strap. The momentum carried him forward, allowing him to slide his right wrist along the sharp edge of the roof strap. Laceration requiring 6 sutures.
Employee was working on an D-11 dozer putting the track together when his chain slipped causing the track to strike his knee.
Spark from welding fell on pant leg. Employee has no feeling in this leg due to a prior injury and did not feel it. When he discovered it, he did not feel it was a problem. Later he developed an infection.
While cutting metal with air arc welder, a piece of hot metal fell into his boot causing a burn. The burn subsequently got infected.
Injured was tramming a portal bus through the outby set of double doors when the trolley pole came off of the wire and swung around and struck him on the right side of the head. The trolley pole limiter was not in use.
Employee was raising the lid on a battery when his right hand got caught in the handle. The lid continued to open and fell against the other lid smashing the tip of his right middle finger.
EE was putting up rib bolt and the rib rolled off hitting left leg.
Employee was trying to get a knot out of a miner cable when his back started hurting. Employee continued to work until 11/2/07 when he went to the doctor for an exam.
EE was carrying a tail pulley with another EE. They were putting the tail pulley in back of foreman's pickup truck bed. While putting the tail pulley back in the truck bed, EE slipped on another tail pulley twisting his ankle.
Employee was loading parts and supplies, tripped and fell with a hydraulic boom jack in his hands. When he fell he caught his finger between the jack and the metal frame of a Mac 8 transport buggy resulting in a fractured fingertip and cut.
Miner was shoveling under conveyor. He threw a shovel full of dirt and experienced pain in the lower back. No mining equipment was involved. The miner was trained in these duties and had been observed to be proficient in this task. All protective equipment was in place at the incident location.
Employee was hanging a curtain from the power center when he slipped & fell off of the power center.
Employee reported to mine management on 04/02/07 that he re-injured his back while placing an oxygen cylinder in an outside rack. Employee has a history of bulging discs from a previous injury in 1997.
EE was trying to reinstall a drive shaft, when he got his finger inside the u-joint, causing his finger to be cut. Lost time injury.
EE complained of back discomfort while operating 400 Komatsu Excavator. He stated seat was uncomfortable. EE finished shift and then sought medical treatment on 3/27/07 for back pain.
Was putting a bucket of mortar over a stopping wall. Being submitted due to Part 50 Audit.
EE was pulling belt back, stepped in a hole and twisted right knee
Employee was reaching over a piece of equipment and felt a pull in his left knee. He continued to work and continued having pain in his knee. He went for a MRI on 4/20/07, had a positive result and began missing work on 4/27/07 due to surgery on his knee.
EE was walking from the section power center when he stepped on a slick piece of rock laying on the bottom causing him to twist his right knee. He was told to stay off his leg for a period of 1 week by medical personnel.
On 3-26-07 EE was getting out of a truck and his foot got hung up, he fell to the ground injuring his back. He continued to work with back pain through this time period with no other incidents. On July 18, 2011 he had surgery which has been accepted as an industrial injury. The difference of the first day off and report date was pending a medical report.
Man was pushing brush tops with dozer. A limb got in a bind and sprung back striking him in the stomach and right leg. Had some contusions and scratches.
EE had been changing oil in an excavator & was latching the hood when he stepped in oil, & his feet slipped out from underneath him, causing him to fall approx 7'. The EE landed on his feet, but did, however result in an injury to his left knee.
while roof bolting in 3rd north development section which has a height of 56 inchs operator strained low back muscle while installing roof bolts.
Employee was using a grinder while another employee was welding. He felt like something was in his eyes and he went to the hospital emergency room.
Employee stated that he had driven a Ramcar to the battery barn to change batteries. As he was traveling into the barn he ran over a concrete block and it threw him upward and he hit his head on the canopy. Started missing 4/02/2007
The employee stated he was moving a shuttle car to the outside and the shuttle car had knocked a couple of the header supports and had fallen down blocking the wheels. He moved one of the supports and the next day reported that he had pain in his left shoulder. He continued to work until 6/25/07. He had surgery and started missing on this date.
Employee was carrying a six foot ladder back to storage area. Employee tripped, kept his balance and did not fall, but injured his lower back.
The employee was stepping up onto the boarding ladder on the 545 haul truck when a pain was felt in the lower leg. The employee was using 3 point contact. There was no rocks or tripping hazards in the area. There was no prior cause leading up to the incident.
EE was installing a roof bolt into a drilled hole. He initially had the bolt head in the machines rotation chuck, when he raised to the roof, the boom head would not reach roof. Lowerd boom, inserted wrench and re-raised. To align wrench on bolt head, he engaged machines rotation while holding onto resin bolt. A burr caught glove and tightened glove around fingers.
EE was cleaning rock from under sub main east 3 belt and strained back.
EE was using a pipe wrench & pulled his left side.
Employee injured back (severe lower back strain) on 3/23/2007 while helping change center cable on superior crane. Employee went to doctor for examination of back on 3/31/2007 and had a follow up visit on 4/3/2007. Employee restricted from returning to work.
Employee was getting ready to advance the roof bolter using the inch trams. As he placed his right hand on the inch tram controls, a piece of draw rock 4'W x 3'L x 1-3"" thick fell striking him on the right forearm.
EE stated while installing 3' resin grouted bolts as belt hangers, the bolt was 1/2 way in the hole, the wrench slipped as EE went to grab the bolt. His left pinky finger was caught between the bolt and wrench when the bolt fell from the hole. 3 right section #3 entry #1 block.
Employee was hanging curtain on #2 section when a piece of rock 2' wide, 4' long, 8"" in the middle to 1/2"" thick on all edges fell out striking employee.
Employee was driving haul truck and was nauseated by the odor of burning wires.
Fire in the preheater unit. Employee, in a rush to prevent injury from the fire, dropped 6' off ladder to lower platform. Employee suffered the following injuries: lumbar strain, knee/leg strain, cervical strain and contusion of back. MRI disclosed no permanent damage. Employee is expected to make full recovery; return to light duty as of 4/2/07.
Emp standing 5' away from borehole pipe waiting for shotcrete line with air blowing through to shut off. Line broke at top of borehole - falling - one of the joint pipes hit employee in head/face.
Rolled water truck - Right stomach contusion, body bruised, especially where seat belt crossed chest, bump on head.
Accident was reported on 3/23/07 as a strain to lower back will driving haul truck. Employee continued to work his regular job hauling mud until 4/3/07 when he returned to work that morning at 8:00am with a doctor restrictions until the 12th of April.
A rock slid & struck the inside of the bed. EE had on his seat belt & the belt pulled on the stitches from non-work related hernia repair some 7 weeks earlier. He was taken to hospital for evaluation. Dr saw no significant damage to repair but suggested follow-up with initial Dr. Only time lost was due to wait to see Dr. The back complaint is new. Citation will be contested.
Miner bumped his head on a cross beam while closing a slide on baghouse cone. Strained neck. Miner was wearing all proper PPE.
Employee was removing a rock from under a 16"" hdpe pipeline with a shovel. Ground had been softened by recent rains. While applying force on the shovel, it rotated, twisting the employee's leg. The twisting force caused the employees kneecap to rotate approximately 45 degrees coming to rest on the ouside of his left leg.
Employee reported on 3/26/07 that he went to the dr on 3/23/07 because he had some pain in his groin area for the last couple weeks.
Injured grasped the handles on scoop canopy to get out of operator's compartment, slipped on floor that was slick due to calcium transferred from roadway & strained left chest muscle & left side of neck. He began losing time his next scheduled shift. *EE had rotator cuff surgery on 7/6/07 for a degenerative condition that was later determined to be injury from the accident.*
Employee was operating the left side of a dual head bolter when his glove tangled in the steel injuring his left hand.
He was cleaning with line shutdown, then he went in & started the line & went back out to clean the tail roller & not sure how he got in the tail roller. The guard was off at the time of the accident & had been off for quite a while.
Injured was empting 55lb. bags of powder mix. He got powder on both forearms, causing chemical burns.
Put start against top, and top broke landing on canopy and then stirking ee in lower back.
While splitting stone, he knelt down and pulled a stone toward his body. He felt a sharp pain in the center of his shoulder blade. He leaned back and felt pain in lower back and felt nauseous. Worker was wearing back support brace.
The jaw crusher became plugged, 2 ee's were working to unplug it. Injured ee was using a bar to dislodge rocks. He lost his grip on the bar and it struck him on the right cheek. He applied an ice pack to the wound and initially thought he would return to work. After a short time, he decided he should go see the doctor. The doctor found he had broken bones in his cheek & jaw.
Walking under conveyor, he stoop up and hit hard hat on roller; sharp pain in neck.
While using comealongs, they slipped and struck employee in the right elbow.**Employee did not start losing time until 5/21/07 due to surgery.**
Traming buggy up entry hit a hole in roadway. Felt pain in lower back.
While riding the mantrip a rock fell and hit ee in the ribs. It took about 10 feet before the mantrip could stop. When ee exited the mantrip he complained about his ribs hurting.
Injured was working on razor knife, cut his left thumb.
Injured was carring two buckets of plaster when he tripped over rock strain back.
Employee was attempting to straighten a pallet of cinder block by using the bucket of scoop and as he backed up the bucket of scoop came off pallet causing the scoop to bounce. His hard hat came off and he struck the left side of forehead on canopy of scoop resulting in a laceration.
Employee was handling lifting straps, twisted left wrist resulting in the need for surgery at a later date to repair tendon/ligament damage.
Injured was bolting top when: a piece of rock fell between bolts and atrs striking canopy the sliding down striking injured.
Employee was picking up the end of a tow strap when he felt a pull in his lower back followed by slight numbness in right leg. Doctor visit revealed a pulled muscle in lower back.
Employee was prying with a slate bar when he felt a pop in his right shoulder. Date with specialist on 4/3/07 states he has a torn rotator cuff.
Employee was getting out of the shower and slipped in soap that covered a large area of the bathhouse floor.
Putting drill pipe on the head, while shouldering up, drill pipe grabbed the rope and started spinning, wrapping him up with rope.
Four employees were trying to remove a copper dogbone from an electrowinning cell, when they lost their grip on the dogbone and it fell, smashing one of the employees foot. Employee was taken to the clinic for observation.
Employee stated that while taking the radiator cap off the generator, steam came from the radiator causing burns to the right arm.
EE was using pork chop clamp & chain to remove belt from storage unit. In the process of pulling the belt the chain broke & pork chop clamp struck him on top of left foot.
A rock was thrown and hit employee in the foot, a minor fracture of the foot resulted.
He was holding the hose from the loading machine and rockdusting out to the crosscut. The loading machine caught the miner cable and hose that was hanging thru the intersection. The hose and cable came out of the hangers and struck him on the side of the head and pushed him into the outby rib. This caused a concussion and strain.
Co-worker was lifting plate to put back in jaw. They thought a weld was cracked so he set it back down. The weld broke & EE's foot was caught. EE pulled it out. 911 called & came - took him to hospital with right mid-foot compound fracture tear.
The employee was walking from the employee building to the shop on relatively flat ground covered with base material. The employee stumbled and fell. The employee's right ankle was fractured along with fractures to her left fibula. These injuries may be further explained by employee being diagnosed with osteoporosis.
The Safety Department informed me, on 3/21/07, that EE was walking in the #4 entry on Sliding Hill Mains, when he stepped into a section of uneven groung and twisted his left knee.
The EE was bringing the mine loader to the office area. As he bent down, he slipped on the stairs and fell on them. The EE feels lots of pain in his lower back side and his right leg.
Bolting up plant in track entry of coal hill. Drilling outside hole operator side. Reaching drill steel, look at his buddy as the plank kicked. Drill steel came out of the roof and struck his finger.
Employee was working in a loader bucket tray when tray came loose causing him to fall.
The employee stated he was installing a bolt when a brow fell off therib and rolled and struck him on the left leg.
EE, off side roof bolter, was prying down loose top in entry 6/7 crosscut 38. He felt his back pop and states that his left leg and lower back hurt.
The repairman was rebuilding a chute. He had to climb down the 54 inch chute to the bottom to take a measurement. While standing on the chute taking measurements, he started to slide forward because of dirty material lying on the chute steel. His left foot toe caught the side of the chute causing his foot to twist outward away from its normal position.
Employee stated that he was pulling on a cable when he felt ""something give"" in his right knee. Mine floor was wet and muddy. He had been diagnosed with a torn ligament in the right knee.
Employee was walk down hill undergorund on north main section slipped and fell catching self with left arm.
Supervisor was standing by the rib at the Stageloader and a piece of rib fell striking his leg. Cause of this injury was safety awareness. Compliance with rules/regs was a possible factor, protective equipment not a factor, miner proficiency a factor, mining equip/systems not a factor.
EE tripped on rubber mat and fell on his face. EE felt pain in right shoulder. Also abrasions on knees and hands. On 11/6/07, EE had surgery on right shoulder to repair torn rotator cuff. EE losing time.
Incident was to have occurred while this employee operating a shuttle car. The car ran over a crib block and a large rock. Victim struck his head against the underside of the canopy. Cause of this incident was safety awareness, compliance with rules/regs was not a factor, protective equip not a factor, mining equip not a factor, miner knowledge could be a factor.
The individual has been workng the past couple of weeks constructing mine ventilation seals. As a result of the repeated exposure to the concrete being used in the Seal construction process, he was diagnosed with cellulitis.
Miner was cleaning stone from grizzlies when a stone slid from top of grizzlies smashing right ring finger between two pieces of stone.
The welder was walking on a catwalk when he sprained his left ankle causing him to fall through an opening at the end of the walkway. He fell onto the next level injuring his ankle when he landed on metal surface.
Employee attempted to pick up a grease drum without assistance, straining his lower back.
Operating a Fletcher roof bolter in face area, his drill auger stopped up; when lowering the steel from the mine roof, it hung in the chuck of the rotation head. His boom was misaligned. As he lowered the boom, at a certain point the misalignment connected allowing the steel to exit the chuck, suddenly move sideways and strike his right thumb resulting in a contusion to the thumb.
Employee was pulling in cable wire off barge on board to dredge. He stretched out in a awkward position and pulled his back.
Employee states he was operating a fork truck unloading a pallet in the warehouse. While backing out of the pallet rows, he was struck by another fork truck that was traveling from the dock area. He was seen by a physician and released for full duty. On April 30, 2007 he went back to the doctor and was not released to return to work.
While pushing overburden benching down to milldale, employee inadvertently trammed off aprox. 30' edge.
Loading 777D with 988H (High Lift), tire fell in a hole and EE said he hit his head, causing a neck sprain.
The bag rack had jammed and the employee was trying to get it out. The arm on the rack came down on the employees right knuckle. Right knuckle was swollen.
While cleaning crusher, ee strained his shoulder and arm.
Employee, operating scoop, stated he ran over a rock in roadway, jammed head into canopy, causing pain in neck and left arm, worked until 9-21-2007 when doctor removed him from work.
Stooped over onto knee's, pushing chain reducer from one pallet to another. Experienced pain in back.
The maintenance crew was moving a conveyor up the ramp to the top of the tunnel stockpile area. A Kobelco LK 300A wheel loader was used to move the conveyor up the ramp. The chain used to lift the one end slipped through the eye in the bucket, causing the loader to overturn on the employee who was walking on the left side of the machine.
Injured employee was helping two others hang a continuous miner cable. They had started lifting the cable and had placed one rope hanger on the cable to hold it up. One employee pulled on the cable causing the rope hanger to slip resulting in the cable falling on the injured employee striking his neck and shoulder. He was seen by his doctor and continued to work until 7-9-07.
Employee was bending down to take sleeve off water pump. Had a burning sensation in his back and fell to the ground.
Putting air con together, sliding the top piece down on shaft & bottom piece. Pinched left hand ring finger between both pieces.
Employee was conducting an inspection of truck outdoors and slip on some ice and strained right knee. he returned to work and just found out on 4/5/07 that he needs to schedule surgery for torn miniscus. Employee is still working full duty at this time
Lifting a 72 inch bottom cradle and felt pain in his lower back. He finished the shift and worked his next scheduled shift. He began losing time on 3/21/07 for lumbar strain.
When crawling backwards after installing part in feeder, employee felt a strain in his back.
Employee was lifting up on one side of a pinner pot for a trussbolter and strained his groin area. This became a Lost Time Accident on March 28, 2007.
EE stated, that while mounting the loader, he felt a sharp pain in his left shoulder.
EE was walking down catwalk, when he said he stepped on a rock, causing him to twist his ankle.
Employee states he was walking the lines on a shot when he stepped on a submerged rock and twsted ankle. Injury broke ankle in two areas.
The EE stated he was bolting top in the belt entry belt channel. The roof was 9' 10"" high. He started to put glue in the hole and stepped on the drill pot, when he slipped and fell on his right side.
1W#4, 24 X-Cut - Employee was carrying a 54"" top roller in for replacement. Caught feet in a pile of BO hi-line, twisted back resulting in strain to lower back. DID NOT LOSE ANY TIME UNTIL 03/29/07.
Employee stated that he was walking thru crosscut 2-3 toward a sump to see if the 480v sump pump was working. While doing so he caught the toe of his right foot on a yellow hook rope that was hooked to piece of old curtain laying on the mine floor. He fell straight forward to the mine floor, hyperextending his right ankle.
Employee came to work and turned in a doctor's note saying he hurt his back but did not report it to his boss or to me when it happened
The welder was changing out oxygen bottles gauge in the back of his welding truck. When he lifted a protective bottle gauge cover-door to perform the task, the cover-door came back down striking his left hand.
Employee was pulling a drive line out of a loader when he strained his left knee and groin muscle.
Injured was carrying wedges down steps on overcast and lost footing and fell. Type of injury is unknown.
While standing on crib blocks working on miner, crescent wench slipped causing him to loose his footing, and he jarred his lower back
EE and co-worker were laying rails and using a rail drag to lift the end of rail up over the track ties. As they lifted a swung rail, it hit a tee and bounced back onto his right foot.
Employee states he had parked and was getting out of the Uniloader. As he stepped down he stepped on a piece of coal, rolling his left ankle. He sprained his ankle and is off work.
Tech was climbing up back of 637 Scraper between transmission and rear fender to change a pump. Tech had grease on bottom of foot and he slipped and fell backwards. As he turned to catch himself, he fell to the ground approx 4 feet. He fractured his left wrist trying to catch himself.
While loading a crane on a trailer, the crane operator had started to pull in front right outrigger. The operator noticed 2 helpers waving at him and stopped movement. Employee had gone to demonstrate where to put chains to tie equipment down, and accidently got his right hand pinched by a return shaft.
Employee had his rt. foot caught in/between a winch drum and the floor plate, resulting in the steel toe on his boot being smashed. There were exposed bolts on the winch drum that pinched his foot and resulting in a cut/laceration of the rt. big toe.
Employee stated that roads were ice and snow covered. He stated that as he was hauling from pit to cleaning plant. He approached a grader as he was moving through a curve. He stated that as he de-accelerating, the truck started sliding and hit the grader.
Employee fell 7 feet while climbing down ladder at the ball mill (#4). He landed on both of his feet injuring his heels.
Shoulder strain due to drilling. Not immediately reported.
Working on plant repairs, slipped and fell against ladder.
Employee stepped up on trunion - foot slipped and hit knee.
Employee was raising major bogie using a come-along to install rollers. The come-along slipped, allowed the bogie to fall mashing his left hand between the bogie and the dozer frame.
Operator claimed he fell off a grader tire, falling down between tire and frame. This was on 09/13/06. Our Personel Director informed me last week that he now needs surgery. He never missed a day until now.
While putting the north back pressure control valve in service, all drain valves were closed. The 6"" drain valve on the inlet line out of V5 had to be closed with a maul. The line was then pressured up from the outlet. The 6"" drain valve was not closed all the way due to scale build up in the valve. Pressure & material blew out of the drain line hitting Jesse.
Employee was attempting to adjust spotlight on off-loader barge by tossing a rope around the light bracket. During the tossing motion, employee pulled muscles in his back. Employee didn't inform mgmt. that he missed work on 6/25, due to this incident, until 7/3/07.
The employee was pulling a steam hose off of a steam truck and hyperextended his left elbow. The elbow is currently strained and the impairment is unknown at this time.
Employee complained of muscle spasm in his back while operating a 637 Cat scraper in tandem with another operator. Employee was transported to the ER where he was checked out and taken off work. Ongoing.
On 3-14-07 while walking under the jaw chassis, employee rose too soon and was knocked to the ground. He said he was fine but while operating the skidsteer two days later his back began hurting.
Employee alleged that while driving a mantrip he received whiplash. This injury became reportable on 9-20-07
Employee bumped his left knee on a rock while rolling up a piece of curtain.
Employee, motorman had a car off track & was in the process of using air bags & lifting jack to put trucks back on track. He was using a bar to lift wheel over the rail, when the car shifted & fell & struck his left hip area.
EE was removing his drill steel from the hole he had just drilled, when the last piece of steel fell from the hole, stiking EE on the middle finger on his left hand, fractured the finger. He has three lost work days.
Employee was removing drill steel from a drill hole. When he removed the bottom piece, the top piece fell striking him on his right wrist.
Operator was on the kiln firing deck observing the material inside the kiln. His helper sounded the horn on the front-end loader, where at this time the operator has instruction to go inside the control room for safety. The helper was removing hot rock from underneath the firing deck platform. Hot steam came up and burned the operator.
The employee was standing on a rock about 12"" off of the ground loading secondary rounds in a larger rock behind it when his boot slipped off of the rock twisting his left knee.
Pc. of draw rock fell out between belt and struck employee on top of head.
EE was assisting in the repair of a brace on the left side rear view mirror. EE had positioned himself in front of the operator's cab, using a pry bar to apply pressure on the mirror he lost his balance on the bar, slipped, causing him to fall approx 9' to the ground.
Employee was attempting to pull a piece of four inch pipe on the stockpile when the pipe got caught between drip and other pieces of four inch. The employee was then thrown off the ATV which then fell over on the top him.
EE was lifting solenoid blocks to load on cart and deliver to work area on longwall face. While doing so, he felt pain in back between shoulder blades.
While installing roof screen, a rock fell and struck him on the back, between the shoulder blades. He began losing time his next shift.
Went to step off of the concrete slab to the ground (2 inch drop) and twisted his ankle. The doctor stated that he was such a big guy that it didn't have to take much to break his foot. We finally got the M.R.I. results and they stated that it was broken.
Traveling with the air course examiner, stopped to fix a crack in a pressurized air line. Line blew; he fell backwards striking his right arm and shoulder on a pile of rock behind him.
Employee was building stopping and a piece of draw rock fell hitting him on the right side.
Using a slate bar to close the door on a ballast car and the bar slipped. He strained his lower back. Injured continued to work until 3/19/07 at which time he began losing time for a lower back strain.
EE was getting on a loader to load himself and his hand slipped and he pulled a muscle in his shoulder and elbow.
Exiting Cat 7R Dozer, pulled groin area.
Operator walking across pit floor-slipped & fell on left shoulder. They were going to set up pump in pit. Another EE witnessed fall.
Employee was pushing a big rock down his run. The rock rolled over pushing his dozer blade back up. When it slammed back down it caused him to pitch his body forward and hurt his back.
Bucketing belt slope on top of the overcast at 8 chute. Employee was using a 5 gallon bucket. The bucket was about 3/4 full. As he was pumping the slop on the belt, foot slipped and he felt pain in the back.
Employee was walking on a catwalk in a poorly lit area and stepped on a 3"" section of PVC piping that was laying on the catwalk. The pipe rolled and caused the employee's ankle to roll.
Employee was injured while opening a set of steel air lock doors. He opened one door & went on the back side of the other door to open it. While he was pushing on the 2nd door the 1st door blew shut against the 2nd door. His fingers were caught between the two doors during the impact of the doors. The impact caused fractures of the index and middle finger on the left hand.
Employee stated that he was installing 3"" waterline in the belt entry. He was dragging up a piece of waterline and when he placed his right foot down on the ground he stepped on some loose material and twisted his knee.
Employee was lifting a roll of skirting and felt pain in lower abdomen. EMPLOYEE WAS ON OVERTIME (15 HOURS)
After completing rock dusting of the main east belt entry, he was pulling the rock dust hose thru the crosscut storing the loops on the machine. When pulling on the hose, stated his elbow popped. Pain has continued in the elbow, started missing scheduled work days 4/4/07, became a lost time injury on the dat.
Employee tripped over rock, spraining left ankle.
While pulling the cable trucks on the monorail with the monorail winch, the cable broke and flipped down the belt entry striking individual in the face. This caused a contusion to his right eye and lacerations to left temple area, bridge of nose and right cheek.
Employee was dismounting the ladder on the 980G cat loader. When he stepped down, he stepped on a rock or clay ball causing his left knee to twist. He did not report this until the next day at 8:30 am 3/15/07. He was sent for Medical treatment and returned to work that same day.
Employee was stepping down off of a scaler when he stepped on a rock and twisted his right knee.
The miner operator was cutting coal in the #5 entry and was getting ready to start his clean-up run when a piece of draw rock fell from between the roof bolts hitting him on the head and left shoulder. He was standing three rows of bolts from the unsupported top. Employee did not start missing work until 3/27/07.
EE was using a jack bar while re-railing a moble on & experienced pain in abdomen. He con't to work then took off on 4/9/07 for hernia surgery.
Two employees were filling a water tank in the back of a pick up truck, when the water was turned on the hose got away from an employee and he fell out of the back of the truck. He landed on his shoulder.
Alledged incident was to have occurred while this employee was wheelbarrowing rockdust in a wheelbarrow. The wheelbarrow tipped over and she tried to keep it from falling over. Cause of this injury was safety awareness. Compliance with rules/regs not a factor, protective equip. was not a factor, mining systems/equip not a factor, miner proficiency a factor.
Employee was cleaning rock/debris from grizzly area under unit #2 cooler and breathed in dust.
Material broke loose, fell from highwall striking employee on his back & injuring his ribs & shoulder. This was a small amount of material consisting of loose dirt & rocks. Victim was about 16' away from walk when struck. Material clump fell around 45' above.
The employee was operating a dozer. He was crossing a ditch and struck his head on the mirror of the dozer. He was not wearing his seatbelt. He cut his head which required stitches.
Employee was moving a piece of rock off the side of the continuous miner. Another piece of rock fell off striking him on side of Rt. foot. Went to Dr. on 3-13-07 (started his 4 off days on 3-14-07) Started losing time on 3-18-07.
Lifting rocks out of jaw crusher by hand after a discharge belt plugged it. Pulled muscle in back.
HE WAS THROWING OVER MINER CABLE. **STARTED MISSING WORK ON 05/24/07**
Victim was greasing boom of the loader when he stepped backward, missing his step point and fell to the ground approximately 4 feet.
Helping to move belt on o west section. While he was lifting belt structure (stand) he felt pain in his lower back. He dropped belt structure and proceeded to inform his immediate foreman of his accident.
He was moving front axle of a 25x with overhead crane when axle caught and swung pinching employee's right arm against machine.
Employee was digging shot rock and hit a hard spot next to highwall with loader. He had been working in shot since 6:00 PM. He said it hurt his back when he hit the hard spot.
An employee was driving a water truck to water the lower part of the haul road. He lost control of the truck and ran the truck up on the berm on the drivers side turning the truck over on the passenter side where it came to rest. The truck was inspected and it was found an airline had ruptured during the operation of the truck. The employee was not wearing his seatbelt.
Checking dragline, tripped, fell in dragline compartment.
Emp. stated while driving elevator chain pin back to make clearance on chain link section, he felt pain in lower back. Pushing an approx. 30 lb. object with a twisting motion.

Employee was working on the bagging line / sealing bags when he felt a sharp pain in his upper back. Employee tried to work in another area of the bag line but could not continue to work. He reported the injury to supervisor and was sent for medical attention.
A remote discharge boom located at the #2 belt drive broke free from the roof pulling back and to the side striking the victim on the left side of face and head.
EE was bolting roof and caught his left thumb between pot and bolt. This crushed bones in the end of his thumb.
Unit mechanic was stepping through a man door in a stopping. When he stepped through with his first leg he twisted his knee. He continued to work until May 30, 2007 when he began missing work to have surgery.
Employee was dragging a piece of wire mesh along side of the miner in 2-South section. As he was dragging it, it caught on something and when it did, he gave a tug on it. When he did this, it caused him to lose his balance and he started to fall. While falling, he put his left hand down to break his fall and as a result fractured his left wrist.
Operating manlift when it came into contact with high voltage electrical wiring.
Hauling 4000 gallons of water up steep grade, truck came out of gear. Operator, unable to get it back in gear, decided to back truck down the hill. Watching for traffic on the right, he negotiated a turn to the left going backward. He failed to see a high spot on the bank and the left drive tires went up on this spot causing the water to shift laying the truck on its side.
Employee picked up a bucket of bits in #7 1/2 wall crosscut, F-13 longwall section, when the corner of the rib fell causing him to turn away quickly. He lost his balance and fell jamming his right hand into the mine floor. He sustained a dislocation to the right little finger and a laceration to the back of the right hand requiring sutures. Employee was wearing gloves.
He was helping in the work on a bearing on a 275C loader when a piece of metal flew off striking him in the right forearm.
Employee was lifting when he lost his footing and felt pain in mid back region.
Employee slipped off the loader bucket with a piece of 2X1 steel in his hand. The steel smashed his fingers against the bucket.
Employee stepped out of the cab of the ANFO Truck. Not knowing there was a drop off, he lost his footing and fell forward. EE said his back started hurting later in the shift.
Employee was using a hand held electric metal cutting saw to cut a piece of metal. The saw kicked back and struck employee on the upper leg.
EE was walking to feeder to service and said that a piece of rock fell from the roof in X cut 6-7 and it hit him on the lower back.
Employee reported that on 3/12/07 he was lifting high voltage cable at the Electric Shop and felt pain in his right groin. He periodically felt discomfort but continued to work. He was recently diagnosed with a hernia and scheduled for surgery to repair the hernia on April 10, 2008.
EE WAS WALKING THROUGH MUD AND HIS FOOT GOT STUCK TWISTING HIS KNEE. EE HEARD AND FELT A POP IN LEFT KNEE. EE TRIED TO WALK AND STARTED FEELING PAIN IN KNEE AND WAS TRANSPORTED OUTSIDE AT THAT TIME (8:30 PM)
He was washing off the bulldozer with a hose. When he was climbing down he stepped on the wet push arm to the blade. His feet slipped between the push arm & the frame causing him to fall sideways to the ground. As he fell he hit his left fibula (leg) on the push arm & suffered a hairline fracture.
Ice may have built up in the hose due to extreme temperature. Although whip checks were installed on both ends, there were no guards keeping the middle portion of the hose from moving in a violent manner.
Nailing curtain to left rib, coal fell out from top, striking employee on top and back of head and upper back and shoulders knocking him to the ground.
Burn on two fingers acquired while burning explosives containers. Molten plastic dripped on fingers causing blisters. Blisters broke and eventually became infected. Sent to town for treatment, and kept from work for several days. No damage to fingers.
EMPLOYEE WAS DRIVING UP HAUL ROAD WHEN HE STALLED OUT. HE WAS BACKING THE TRUCK UP TO GET OUT OF THE WAY AND WENT INTO THE DITCH.
EE was scooping in old works and scoop bucket caught a piece of 4"" plastic waterline, it sprung back and hit ee in face. Diagnosed with facial fractures. Started missing work 3/12/07.
Mechanic was lifting belly pan back up on dozer and hood on chain became unhooked. Belly pan swung down and hit mechanic helper in right foot. Top of foot was blue in color.
Mechanic, changing belt on 777D rock truck, using 6' ladder against frame, was using wrench on ac compressor adjustment bolt. Bolt broke loose, jarring ladder which kicked out at bottom. Mechanic fell against lifting eye on truck frame. Ladder stopped by truck tire.
Employee was jackleg bolting when his left thumb was caught between the bolt driver and the bolt. This resulted in a broken bone in the thumb and a laceration which required sutures.
Employee was pulling miner cable out of shuttle car roadway and strained his right shoulder.
Employee was checking antifreeze in truck. He started taking cup off of radiator and realized it was hot. He started putting cup back on when it blew off covering him with hot antifreeze.
EE was driving down S400 belt line looking for wood blocks. While looking to the left side of the room, he drove up onto a heaved floor and either struck his head or was struck by something causing a laceration. Stitches were required and missed workdays started on 3/11/07.
8I track, as ee was walking up to the section, he tripped over a tie & fell. He experienced lower back pain & a sore right wrist. *Lost work days began 3/12/07.
EE was helping on an old buggy splice---pulling off tape, when the tape slipped and an opened knife he had in his hand, closed up on his little finger on his right hand requiring 7 sutures.
Truck operator noticed small amount of smoke from dash area of production truck. Appropriate personnel arrived at scene, could not find cause of smoke. Operator went to clinic, irritation of lungs.
Employee caught in the tail pulley as he was cleaning material from beneath it.
Employee was stepping out of forklift, when he twisted his left ankle. This was not determined recordable until medical review on 3/19/2007, employee will be required to have surgery.
Operator was climbing up into the rock truck and pulled a muscle in his shoulder and neck while grabbing the rail.
Employee was hanging phone line. He had spool of phone line hanging from roof bolt plates with nylon cable hangers. One of the nylon cable hangers broke and the spool fell on his right calf and ankle.
Setting timbers and bruised knee on a rock. EE was not wearing knee pads.
The employee was installing a canopy jack on the roof bolter. He placed the canopy jack on the front of the roof bolter and the jack slid off and hit him on the right foot and ankle. This caused a bone fracture under his ankle.
EE was crawling out from underneath a feeder.
Employee loaded some high voltage cable on a skid. He could not recall a specific incident. After lunch, he had trouble getting up from his seat and had a pain in his lower back.
Employee was checking engine oil on rock truck. He was stepping back, tripped and fell backwards into the trucks handrails striking his shoulder and neck. The employee was not taken off work until 3-14-2007.
Employee was stacking rock onto a pallet. When he picked up a stone he twisted toward the pallet and felt a sharp pain in his lower back.
The employee stated that he experienced rt. shoulder pain after handling exhaust tubing. He was diagnosed on 3/22/07 with rotator cuff syndrome.
Employee was operating miner in #6 entry when the rib rolled off, striking him across the hip knocking him down and partially landing on his left leg. The rib roll was approx. 10' long x 3' wide x 3"" thick. This resulted in a fracture to his leg and ankle.
Employee was climbing down a ladder and twisted his ankle on a stone.
Rock fell between bolt and rib hitting employee on left ankle.
While performing maintenance work on a Euclid R-50, prior to Quarry startup, Company ID #H-09, the employee was using a grinder to prep metal for welding of a crack in the body of the above mentioned Euclid R-50, when a metal flake entered his left eye (cornea).
The drill tech was replacing a leaky fitting The drill tech stepped up onto the chain wrench to get a better reach. The wrench moved and he lost his balance. His knee struck the wrench during the fall injuring his knee. Knee strain possible level 1 trauma.
Worker fell seven feet when his glove caught on a piece of metal that was being thrown off the side of the wash plant. Employee was changing a screen in the wash plant without his fall arrestor.
Loader blew a hose, leaked oil, causing a fire. The EE panicked and jumped off loader, injuring his knee.
EE was attempting to get down from the generator trailed by using a ladder. When he put his foot on the top rung, the ladder kicked out from under him. He fell approx 5' to the ground and landed on his right side.
Injured EE was holding onto drill steel with up pressure on head. He was attempting to drop head to change bit end. Steel broke in 2 pieces. Top part remained in roof & other part in drill head. Upward pressure abruptly raised to roof injuring thumb.
Operator got too close to corner and didn't hit panic bar to cut bridge off. Caught operator up against rib, skinned his back in 3 places. Operator wasn't paying attention & wasn't alert as to where he was on corner. Operator was warned 2 times earlier in shift about falling asleep while operating bridge.
Another EE was hitting a dust box with a hammer when a piece of the hammer chipped off and hit injured in his right eye. He was 6' away from the dust box. He came back to work on the 9th and 10th. Doctor took him off work on the 12th.
