MRS was in #4 entry; had finished right run when operator let shield down. Top came down on MRS. This was a non-injury roof fall.
No. 6 Entry between break 30 & 32, 2 breaks inby spad 834. The roof strata was soft shale with a fault. The fall was 75 ft long 20 ft. wide 6.5 ft high.
Roof fall discovered on weekly exam in the 2 West 1st Room #5 entry x-c #2, 30'x30'x8'thick, fall had already been dangered and timbered off before falling, mining is now in the 3rd rooms of 2 West.
There were no injuries or employees on duty at the time of the fire. Kenworth tractor was parked on the parking lot on a Sunday night when a security guard noticed a fire. He called the fire dept but by the time they arrived the truck was basically burned up and fire was dying down. Appeared Electrical.
On April 29, 2007 AM a rock burst occurred at the Galena mine. The morning of April 30, 2007 at approx. 7:35 AM I was notified by the crew of the 226 stope that they had discovered the location of the burst. It occurred in the 226 raise. The crew immediately vacated the area to wait for their supervisor. At 7:37 AM I notified the Boise MSHA office & at 7:47 AM the MSHA 1 Call line.
A roof fall occured in the #11 seam, 1st northeast submain, #2 unit, #5 entry, spad 8+40. The fall is 90 feet long, 19 feet wide and 10 feet high.
Ignition occurred on the Longwall face at No 128 shield while the headgate drum was cutting the roof. The ignition was at the roof of the headgate drum.
An unintentional roof fall occurred on the 8th North Headgate set up Entry #2, Crosscut #41L (1483N), Crosscut #41R (1524N). The fall was of unknown length, approximately 19 feet wide and approximately 10-12 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up.
Four men were on top of cage, approximately 200' below the collar of the #4 shaft removing pipe when a failure of the hoist motor occurred. This failure was caused by a phase to phase fault. The crew was lowered to the 1200 level and then exited the mine via the #3 production shaft.
A roof fall occured in the 3 south panel, #9 entry, starting halfway between crosscut #11 and #12 and ending halfway between crosscuts #12 and #13. The fall measured approximately 75'x18'x9'. The area was supported with 5' fully grouted bolts and 9' double lock mega bolts. An undetected slip was determined to be the cause of the fall.
Wana service cage located at wana lost power with nobody on the cage. The cage had just been lowered onto the chairs when the kellum grip broke free causing the trailing cable to pull loose from it's connection. The trailing cable was replaced, new kellum grips installed and power restored.
Rock fall in #6 entry outby 3 section approx 3500' near spad 410. 60' long X 20' wide X 6' high.
A roof fall occurred in the #9 seam, 2b panel, #8 entry, crosscut 51. The fall is 45' long, 18' wide, 6' high.
A roof fall occured in the #9 seam, 1st southwest main, #4 entry, spad 6+30. The fall is 20' long, 19' wide, 8' high.
On the 084-0 MMU, 11A longwall face near the 153 shield, an alledged ignition occurred near the mine roof at the tail shearer drum. The alledged ignition was self extinguishing. No injuries to persons or property. The WVOMHST and MSHA agencies were immediately notified.
A fall occurred in the #11 seam, 1st northwest main, #6 entry, spad 4+90. The fall is 18' long, 25 wide, 6' high.
Roof Fall very end of 10 Rt. submain at old face of #5 enrty @ B-27, inby return travelway. 20'X20' wide, approximately 4 feet high. Did not affect ventilation. No one injured and no equipment involved. Found by foreman during his shift at 3:40 a.m.
Elevator down on 4-26-07 at 1:00 pm. It was temporarily repaired. Severe rain and weather worsened the condition. Elevator down on 4-27-07. National Elevator was called to repair. Elevator repaired on 4-27-07.
A roof fall was found in the right return at spad 5090 during a weekly examination. The fall is in #7 entry, 70' x 20' x 6' MSHA inspectorate personnel was present when the fall was found during the exam and took the appropriate information. the fall will not be cleaned up
Unintentional roof fall 3rd North Sub Main LOX 8000' mark between #11 and #10 Entry. 20' Long x 18' Wide x 6' High above the coal seam. Area is bolted with 5' Double lock bolts. Proped around fall
Rock fall occurred on the track 1 North Mains between break #24 and #25. Fall measured approx. 8' wide x 20' long x 2'-2.5' thick.
An unplanned rooffall occurred in the #130 mine that measured 60'lx20'wx61/2'h.
Roof fall 8th North Panel, No 3 entry beginning at spad 6+40. Approx 140' long X 8'-10' thick X 18' wide.
At approximately 7:10 AM two employees determined an inundation occurred & one of them immediately called MSHA. Inundation consisted of South Platte River spilling into pond. No equipment or personnel affected.
An unplanned rooffall occurred in the #130 mine that measured 40'lx20'x61/2'h.
Roof fall occured in the No. 5 entry of 2 left mains, 1 break outby spad No. 2249 in an area under construction being rehabilated for the section relocation. The fall was approximately 60 ft. by 20 ft. by 4ft. thick.
A roof fall occured in the 2B Panel crosscut 48, #7 entry, spad 31+97. The fall is 30' long, 18' wide and 8' high.
The face of the No.1 entry (inby survey station No. 1129 was being mined on the mmu 001-0. When the cutter bits struck the sandstone mine floor a possible methane ignition occurred. The continuous miner operator saw an orange flash that immedimely self extinguished.
Unintentional roof fall discovered in the non-producing #1 Section, #1 entry 1 block outby station 4881. The falll measured approximately 100'L X 18' W and the height is unknown. The area was previously supported with 6' fully grouted resin bolts. The area was mined between 8/3/06 and 8/8/06. There is 340' of cover.
A roof fall was discovered in the 2 North 3 Mains, #2 entry, crosscut 17-18. The fall measures approximately 70' long, 18' wide and 6' thick. The area is supported with 5' fully grouted resin bolts.
While the slope hoist was running the falk coupling broke on the main shaft. MSHA was notified at 8:30a.m. Hoist was repaired at 3:25p.m.
A cable/utility man was at the unmanned ES42 shovel, preparing to move the spanners when he noticed an orange glow and upon inspection found a fire. From the time the fire was spotted to extinguished was noted as 29 min. MSHA was given a courtesy call as it had likely burned longer than 30 minutes. A ring tail was discovered in the gear body, and is blamed for causing the fire.
On 4/23/07, an unintentional roof fall occurred on the 1st North Panel off the 2nd West Sub Main off the 4th North Sub Main. This roof fall was supported with 6 point anchor roof bolts. The roof fall measured 30' long X 13' wide X 2' high. The roof fall will be cleaned up.
A roof fall measuring 40'L x 18'W x 7'H occurred in the intake # 6 at spad No. 1866 in No. 2 SE Mains. Area was bolted with 6' TT bolts.
Distance from active face:1400', Airway: neutra, intake (have two intake entries, have dangered one off), Size: L-30', W-20',H-4'; 48"" resin; Outby area had supported wiht 50 ton jacks; strata: shale, laminated; No coal rider seam present, All entrances to fall breakered off, area will not be rehabilitated.
On 4/22/07 at approximately 10:30 pm the preshift examiner discovered a roof fall in North Mains #6 entry cc 33 to 34. The fall was 20'x100'7'. Several small roof coal falls were discovered in adjacent entries.
Contracting company M & W were drilling a hole into an abandoned mine for slurry ejection. While removing the drill steel from the hole, the drillers encountered methane and an ignition occurred. One driller sustained miner burns to his left arm.
A grass fire was noticed on the north side of the RR tracks approx 200 yards east of Oak Hill coal silo. The mine water truck & local fire dept., extinguished the fire in approx. 35 mins. No injuries, equipment of facility damage. MSHA hot line was called at 1:42pm. Msha ret'd call at 2:20pm. Probable cause; sparks from coal train.
An unplanned roof fall occurred in the No. 3 entry of 1 Right Room at Survey Station No. 639. Roof fall was approximately 19' wide, 45' long, and 4' plus high. Area was bolted with 4' tension rebar bolts according to the approved Roof Control Plan. Did not affect any equipment, ventilation or travelway. Area was timbered and dangered off.
A methane gas inundation occurred on the active pillar line of the 4-left section due to a pillar fall inby the pillar line in the active gob.
A roof fall occurred on the 3 Right section track and belt entry. The fall was approximately 30' long by 20' wide and 6' high. The fall will be cleaned up and will be resupported.
A roof fall was discovered at approx. 7:10 AM in the J-3 Tunnel Mine at #1 break. The fall was approx. 10 ft. wide by 50 ft. long and above anchorage. The area was bolted with 4 ft. fully grouted resin bolts. The area has been dangered off and heintzman Jacks and cribs have been placed at access. The area was developed in 1992.
A roof fall occurred at spad #1868 in the #7 entry outby the working section. The fall was 8' high x 25' long x 20' wide. It was bolted with 5/8 x 72"" fully grouted rebar and spot bolted with 12' cable bolts. The area was dangered off and all sides timbered off.
ROOF FALL (7000-1-003-07) - LOCATION : MW#1 Roadway, between x-cuts #15 and #17. DIMENSIONS : H-6', W-19', L-150'. GEOLOGY : Soft, medium to dark gray shale. ROOF SUPPORT : 6' conventional bolts. The area was mined in 1983. Cutters and draw slate were present in the roof.
A roof fall occurred in the 1 Main East in #5 entry in the intersection of cross-cut #23 and extending through the pillarline for 20ft. toward #6 entry measuring 18ft. wide x 40ft long. x 8ft. high. The area was supported with 6ft. tension rebar bolts and 9ft. mega bolts. An undetected slip was determined to be the cause of the fall. This area was mined in April 2004.
A roof fall occurred in the #10 entry inby spad #1866 which is outby the working section. The fall was 80'long x 17' wide x 8' high. The area had been dangered off prior to the fall. The area was bolted with 5/8 x 6' full grouted rebar and spot bolter with 12' cable bolts. The area was timbered off on all sides and dangered off.
A roof fall , approximately 40 ft. y 20 ft. by 4 ft., occured about 8,500 ft. outby section at spad No. 86.
Roof fall, 7th subpanel north, #3 unit, belt entry at spad 52 & 30. approx. 80' long, 18'-20' wide 6'-10' high.
A fall of roof occurred in the 1D Sub Main at cross - cut # 27, in the # 2 Entry. The fall was approx 65ft long x 18' wide x 6' high.
Hill seam it was just bad top. Will set more cribs so it won't come on any father. it is about 20' w x 40' long x 7' h. It is at @2 xcut on #5 belt in between 2 block between #1 & #2. Found it coming outside at 3:30pm.
A fall of roof occurred in the 1A sub main at cross cut #6 in the #6 entry. The fall was approx 50'long x 18' wide x 6'high.
While advancing a crosscut left off #4 entry of mecharized mining at #00-0 near survey station #1228 a 3"" borehole was cut into. Mining operations ceased until the hole was plugged and a 103(R) order issued by MSHA was terminated. There were no injuries and no property damage. MSHA national hotline was notified.
A fall of roof occurred in the 1B submain at crosscut #4, in the #6 entry. The fallwas approx 45' long x 18' wide x 6' high.
A fall of roof occurred in the 1st North West Mains in the #1 entry between cross cut 164 and cut 165. The was was approx 25'long x 18'wide x 6'high.
A roof fall occurred in the 2nd West Submain, Crosscut 81, Entry 6. The fall was 20' long, 20' wide and 8' tall. The area was bolted with 6' & 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.
An unintentional roof fall occurred on top of a Joy 14-15 continuous miner in the face of the #7 entry of 4-Left approximately 30' inby SS# 3813. The fall dimensions were approximately 20' L x 20' W x 6' high.
At or about 3:10 pm power was lost to the mine amd some of the surface facilities due to power loss from the incoming service line. The power lost effected the Main Hoist and stop operation until line was repaired for approx. 2 hrs.
A roof fall occurred in the 1 Main East in #4 entry in the intersection of cross-cut # 56 measuring 18ft. x 18ft. x 6ft. The area was supported with 6ft. tension rebar bolts. An undetected slip was determined to be the cause of the fall. This area was mined in August 2005.

Unplanned roof fall occurred on Break #6 on #10 belt 2 bks inby spad #7379. No injuries or equipment damage due to roof fall. Roof fall measured approximately 25 ft long, 20 ft wide, varied in height from 0 to 6 feet. Area was barricaded off to prevent travel.
Had a rock fall - 60 foot outby spad 603, 20 ft wide x 50 ft long

A fall occurred in the # 6 entry of Southwest Mains, 1 break inby spad # 1452. The fall goes through 6 right into 7 right breal. This is an abandoned area of the mine. The fall is approximately 10' to 15' high and 120"" long. Was discovered today. Actual date of fall is unknown.

A continuation of a previous roof fall was discovered during a weekly exam. The fall was discovered by company and MSHA officals. The fall was located in the #4 entry x/c 36 of the East Mains. The fall measured approximately 8 ft high, 16 ft wide and 20 ft long. The fall did not impede passage of men or impair ventilation.
Roof Fall North Submain Break#40 #5 enrty diagonal across intersection approximately 4 feet high, (just above bolt anchorage), 10 feet wide and 25 feet long. Did not affect ventilation. No one injured and no equiptment damage.
Fuel tanker suffered mechanical failure leading to fire and explosion of tanker. No injuries were sustained.
There was a problem with the Production Hoist so it would not operate. Discovered a blown fuse on main number 1 that was replaced. Changed the mother board in drive number 1 and repaired an air leak on drive number one cabinet. Hoist became opertional again at 3:00 p.m. on 4/17/2007.
An accumulation of water in the #2 entry at varying locations from #5 break inby to #40 break prohibited the use of normal mine tranportation in this area. A new pump discharge line was installed.
An unplanned roof fall occurred in the winifrede mine that measured 50' long x 8' thick and 18 1/2' wide.
A Fall of the roof occured on the 6a belt x-cut #7 of the 3rd N.W. subpanel, supply road. the fall measured 60 ft L, 20 ft W, 7ft H, Ventalation. Equipment or Personell was not affected.
A fall occured in the #11 seam, northeast mains, #1 unit, #4 entry, between crosscut 7and 8, between spad numbers 4+90 and 5+60. The fall is 50' long, 18' wide and 10' to 12' high.
Heavy rain flooded section of mine covering bolter and shuttle car with water. No reportable injuries.
A roof fall has occurred in the second southeast mains in no. 5 entry at spad 25+20. The dimension of the fall is 40' long, 18' wide and 12' high.
While mining into the headgate a bounce/bump with floor heave occurred, causing damage to the longwall shear, resulting in mining being impeded for more than one hour.
Due to flooding conditions due to excess rain the seven foot drain pipe under the mine yard stopped up and water entered the portals. Resulted in water roofing at the #2 belt drive blocking all entries for travel.
Top in break #35 inby spad #243 had been cribbed off for 3 weeks and dangered off. On 4-15 top fell in at break #35 about 40' long, 19' wide, 4' on left side and 6' to 7' on right side.
The Oakdale portal elevator was inoperable from 7:52 AM until 9:35 AM. No entrapment - No injuries. Schindler's Elevator Service was called to the site and repaired a door closer slide. The elevator was returned to service at 9:40 AM.
Fall of roof #5 entry, 1 br inby spad 497. Date mined 4-15-06. Slicken sides, longwall pressure. 3000 feet from working section. No airways or escapeways were affected. 9 cribs and 16 timbers were set around the fall.
The continuous miner (Joy) was mining in the #5 pillar block. A roof fall from the pillar line fell catching the ripper head on the continuous miner. No injuries, no blockage of ventilation, no blockage of escapeway. Fall not cleaned up. Breakered off & dangered, plotted on map.
A roof fall was found by one of our belt examiners along the 5 West A belt at crosscut 29 between entries 3 and 4. The fall was located adjacent to the 5 West A belt line. The fall was located in the crosscut and did not extend across the intersection.
Hoist car was down due to main electrical breaker failed.
Highwall gave way onto the canopy and blocked the track entry. Fall was 3 to 5 feet long, 18 feet wide and 5 to 6 feet high.

A roof fall was found in the 6 Left, 1 Left - 4 Main North worked out area by our weekly examiners. The fall was located near the area where our examiners take their air readings. The cause of the fall was weak laminated shale and slips are also present in this area.

Roof Fall that started at the Face #6 Entry 3 West Main STA#6103 extended initially 125', over the next 2 1/2 shifts Fall travelled outby on diagonal direction approx. 600' to #2 Entry STA 6001. 7 pieces of equipment (2 Miners, 4 S Cars, 1 Feeder & a Section Ld Cntr)inby on the right side were retrieved after the area settled down & several areas were supported by cribbing.
#4 entry x-c32, area already known as bad top and had already been dangered off, fall found during AAA inspection. 40'x 40'x 10' thick. Area has been cribbed off.

A coal mill explosion occurred within our coal mill. No injuries of plant personnel. Contributing factors were a diff. of about 20F between control room thermocouple and manual readings. Operational set point for coal mill exit temp. is about 180F and with this diff. there was the possibility that the temp reached 200F.
A roof fall, approx. 60ft. x 20 ft. x 6 ft. thick occured in the No. 6 entry 2 breaks outby spad No. 121, approx. 8000ft outby the section. The fall has been dangered off and breakered off.
A roof fall occurred in #5 entry, #11 crosscut of the 1st North Submain. The fall was 20' long, 20' wide and 8' tall. The area was bolted with 6' & 8' point anchors. The fall didn't affect ventilation, travel, cause injury or damage equipment.
2 South, existing roof fall extended from return #2 entry x-c19 on over to #1 entry 15' x 12' x 8' thick, area was cribbed and dangered off. this was part of secondary escapeway, 2 south section was idled until secondary escapeway routed over thru #3 entry x-c 19.
Roof fall occured int he track entry at x-cut 8 at spad no. 40 on thr rh1 trackway. Fall was approx. 4' thick x 20x30. Area will be resupported and cleaned up.
EE was fueling up a welder when gasoline splashed on the hot manifold and ignighted the welder which was mounted on a truck. The welder and a 1989 Ford 350 4X4 pickup burned to the ground.
3 EE's became stuck in elevator.
A roof fall was discovered in the 1 North Mains, Crosscut 30, #8 entry. The fall measures approximately 70 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts.
3rd Shift Fireboss was doing preshift for Sunday night when fall was found in belt entry #3 entry Across from spad #1451 fall was 4' to 6' thick, 30-40'longl Fall was outby the section.
The Miracle Run elevator doors failed to open while the elevator was at the surface landing. The Shindler repairman installed a new motor and relay switch. The elevator was returned to service at 12:30 pm.
A fall of the roof occurred on the #2 unit in the 5th N.E. panel x-cut #14 in the last open x-cut. The fall measured 80 feet length 20 feet width 6 feet thickness Personell, Ventaliztion or equipment was not affected.
The roof in the crooscut between spads 6268 and 6258 fell above the anchorage. No person or equipment were involved. Mining was recently stopped in the area and no work activity was taking place at the time of the fall.
An unintentional roof fall occured at the 20+50 station, entry #6 on the 2B return. The fall did not affect ventilation or passege of men. The fall occured at an intersection and it measured 22foot x 20foot x 6foot. The intersection had been supported with 6 foot fully grouted roof boltes. All approaches have been timbered.
5 Mains 39 block, #1 entry 4 way-MMU 003. 25' long X 18' wide X 8' high. Support: 6' 3/4"" resin bolts, 12' cable bolts, pans, brow straps and truss bolts. Fall dangered off, additional roof support installed around fall.
Fall was outby intersection for a distance of 25'long x 10' w x 5'high. Intersection had cribs and headers previous installed but had not indication of this fall outby cribbed area.
An unplanned roof fall occurred at spad 34. Fall area developed December 2006. Approximately 4000' outby the working section. Fell in #2 entry. Fall measured 23'W x 30'L x 8'High. No accidents reported.
Problem reported with Koepe mancage. Investigation began and found bottom stop switch failure. Evacuation started, but stopped. Mancage operable on manual mode at 6:05pm. No injuries; no entrapment.
8th panel north, #3 entry neutral. Approx. 45' long x 18' wide x 12' high.
Roof fall in the intersection of #4 entry 008 section at the last open break along anchorage. Ventilation not affected. No injuries.
Excessive temperatures caused an unplanned ignition of two non-electric detonators in the Kiln 1.
An unintentional roof fall was discovered in the #3 entry at spad #793 at break #29. Size of fall was estimated to be 5'H X 18'-20'W X 60'L. Fall will not be cleaned up. No injuries occurred, no equipment caught and ventilation was not interrupted. Area had been bolted with 4' fully grouted resin bolts. Fall will not be cleaned up.
A unknown type fire started in a screen deck during maintenance which spread to 2 conveyors and 2 additional screen decks. The fire was extinguished in approximately 40 to 50 minutes by local fire departments. There were no injuries or illnesses resulting from this occurance.
Was getting the 2nd run in front of block between the #5 & #6 entry. When the miner head cut thru a piece of rock fell on the head pushing bits into bottom lodging in the miner.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to the counterweight activating the buffer switch. Schindler was called and the cables shortened the buffer switch repaired. The alternate escapeway for D-Mains was rerouted to the slope. No persons were in the elevator at the time.
An unintentional roof fall was discoverd in the #1 entry (return) one break outby spad #816. Size of fall was estimated to be 5'H X 18'-20'W X 20'L. Fall will not be cleaned up. A roof fall was discovered in the 2 North 5 Mains, crosscut 25, #2 entry. The fall measures approximately 30 feet long, 20 feet wide, and 6 feet thick. The area is supported with 5 foot fully grouted resin bolts.
A roof fall was found by our weekly examiner as he examined the 3 Left - 4 West intake air courses. The fall was located at crosscut 32 between entries 7 and 8. The fall was located adjacent to the primary escapeway for #5 unit.
An unintentional roof fall was discovered between #2 & #3 entry at spad #785 at break #28. Size of fall was estimated to be 5'H X 18'-20'W X 55'L. Fall will not be cleaned up. Coal was being mined in the face of the no. 3 entry (about 70' inby survey station no. 1090)on the mmu-001-0. When the cutter bits of the continous mining machine struck the sand stone mine roof. the machine operator saw a orange flash on the right side of the machine's cutter head.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON THE 2ND WEST TRAVELWAY AT THE INTERSECTION OF THE NO. 5 ENTRY AND X-CUT #30. THE FALL IS APPROX. 50' L x 18' W X 8' H IN DOME AREA.
The #6 elevator drum brake failed to release when five people were on board. The elevator was down from 10:45 until 11:05. Cumberland coal resouce l.p. does not believe this incident meets part 50 requirementf for reporting but we are sending this 7000-1 report under msha direction.
7000-1 form submitted solely for the purpose of terminating citation # 6385345 and should not be construed as admission that Newmont violated any MSHA regulation or that it concedes that a reportable event occured. A fall of ground occured on 4/1/2007 in the SW 5540-562 of the Carlin East Mine. The total fall was approximately 2 feet thick.
A coal rider seam 1"" thick that was 2' above test hole length.
The #7 main hydraulic pump on the Hitachi EX3500 excavator had a catastrophic failure. Evidence shows that the pump exploded and started a fire. The fire began near the right engine compartment and spread quickly. The operator was able to safely exit the machine.
The truck operator was traveling from the 870 level of the quarry to the waste dump with a load of waste material. The truck left the road, overtraveled a berm and continued down the highwall, rolling over before coming to rest int he quarry. It is believed that the truck operator suffered a medical event and as a result was unable to control the truck as it left the road.
Other ee was contructing a work scaffold, 5th floor, Bldg 450. Another ee was at the 10 foot level passing material to the top floor. While EE was re-locating he fell app. 60'.
Employee fell from cab of drill when drill tipped over. Employee fell approximately 40 feet from the dab to the next lower bench. Fall resulted in fatal injuries.
A highwall measuring approx 150' long, 180' high, & 75' wide collapsed resulting in fatal injuries to 2 miners that were working beneath the highwall. One was operating an excavator & the other was operating a bulldozer. Examination of the remaining highwall indicates the highwall failure originated at the toe of the columnar section above the Pittsburgh Big Vein coal seam.

Operator hired contractor ee for the first time to replace tires on a Case 721 wheel loader. He was to supply all parts, tools, equipment, and labor. Experienced miner escorted contractor to the wheel loader. Contractor did not inform operator of any hazards that might be created by his works. He was underneath loader when the loader fell off two jacks onto him.
The victim was standing behind the center bolter observing the slack power cable laying on the ground. As he walked around the bolter to check for slack, he stepped on a small rock (saucer dish size) causing him to twist & fall to the bottom. He was taken to the hospital and a soft cast was put on. He went into convulsions on 4/28/07 at home and later died at the hospital.
The worker was in process of adding Material to the base of the tailings dam at Brushy Creek Mine. The moxy haul truck was going down the slope of the dam when the driver lost control. As he tried to negotiate the turn near the bottom, the truck rolled over and landed on side in a 6' deep seep pond. Diver's body was found pinned under truck and in water.
Employee was helping mechanic move some 10"" x 10"" x 4"" wood blocks that was blocking tires while he was using to chock wheels. He said that he felt something pull but did not think it was anything until next day. Sent him to the Dr. They said pulled muscle. Then on 5/16/07, he went back and they found a hernia.
Crew was trying to get engine on drill to start. They pulled driveline to pump box and at same time pulled guard that covers driveline & alternater. For some reason, inj EE reached down to check tension on alt belt at which time engine turned over, catching his finger between belt & pulley.
Employee was working on the miner under a cover about mid way of the top of the miner. He needed access under an adjacent cover & it would not open. He stood on top of the miner to pry open the adjacent lid when he slid off the miner. In an attempt to catch himself he grabbed the cover that was open & inadvertently pulled the lid closed amputating the end of his right thumb.
Injured employee attempted to remove the build up off the discharge chute of the Urschel polymer slicing unit. Employee's right hand index finger was severed by the feeder auger. Injured was then transported to emergency room.
Lifting a pipe with a chain hoist, chain slipped and the pipe smashed the left ring and middle fingers. Employee had choice to have finger grafted to chest for three months or have amputated at first knuckle, Employee decided to have middle finger amputated at first knuckle.
Employee was placing the dishcarge trunion bearing into place on Mill #6. He was trying to remove a piece of cribbing when the cribbing shifted pinching fingers on his right hand between the cribbing (wood) and the casing on the mill. Resulted in laceration and partial amputation of mid and ring fingers of right hand.
While placing a sheet of steel to be welded as a bed liner into the R90 rock truck, the employee felt a strain in the groin area. After several days a bulge developed. Upon examination by his family doctor it was found that he tore his groin muscle.
Employee was moving a steel beam manually with his hands. The beam shifted and slid onto another beam pinching his left pinky. The resulting injury was a 1/2"" amputation.
Hand pinned between fork and shield
EE's left boot was caught between the shuttle car and the coal rib. The toes were mashed severely and some appear to be broken.
IE severed left index finger while assisting in the removal of hatch door in the pre-heater tower.
Injury sustained was severed right index finger. Patient underwent surgery to repair finger. Patient is still in hospital for observation and treatment. Injured and another EE were replacing a new alternator and fan belt at screen plant, the guard had been removed.
Employee was placing a suction floater on the tongue of the pump trailer. The floater was upside down and he attempted to flip it over. As he did this the floater slipped and when he attempted to grab it the metal box the hose hooks to swung around and cut off the end of his right index finger. He was taken to a physician and couldn't return for his next shift.
Tank truck top loads from a fixed platform with a hinged ramp to the top of truck. Upon completion of loading, employee pulled the ramp up and gloved hand slipped causing him to fall back 2 feet striking the safety rail brusing low back.
When climbing down steps to crusher work platform, slipped off bottom step, fell landed on L-knee, reached out to break fall caught L-small finger.
Employee was lifting a ladder down from a rack and twisted the wrong way and felt a pull in his back. He was evaluated on 5/2/07 and was taken off of work.
Employee strained his back while moving a large rock.
Employee was operating a dozer when he backed over a rock and strained his neck and shoulder.
He was pulling down loose top when a piece of rock came loose, struck the ventilation tubing, the rock bounced off the tubing and struck him on top of the right foot, causing a contusion of the right foot.
Bruised hand tensioning bolt. The worker did not mention the issue until the next morning. Office dispatched him to medical. Released with hand bruise (splint/ice) to modified duty pending specialist review. Office recommended off duty pending specialist review next week.
Employee was kneeling down chiseling end of stone cube when his upper left arm was pinched between the two stone cubes.
Employee was helping pull a piece of belt on to beltline. He slipped and fell over belt rail.
While installing supplemental roof support a piece of rock fell from the mine roof striking employee on the head, left hand and lower back. Employee received 5 staples in the head and 3 sutures on the left hand.
The EE was loading plates when he turned to throw a bundle onto the roof bolt machine and experienced a sharp pain in his lower back and numbness in his right leg. The EE worked through the following Monday (5/7/07) and began taking off work on Tuesday (5/8/07) as a result of this injury.
Employee had finished shift, parked drill and was climbing down from machine. When he stepped on ground and released grip on ladder, he twisted right leg. Employee did not insure proper footing before releasing grip on ladder.
EE was grading road in 2nd gear pulling the ditch back across road. The blade hit a rock causing him to slam forward & hurting his back. He called his foreman on CB radio to come to him. When foreman arrived he asked EE if he needed an ambulance. EE said, ""No, I can drive myself to Dr's office."" Foreman asked him if he was wearing seatbelt. He said yes.
Stuck two fingers in air conditioning vent hole, damaging two middle fingers on right hand.
Propane leak. Gas buildup under crusher feeder. EE changed propane tanks w/o turning off the burner valve which caused a build up of gas when he lit the burner it caused gas build up to flash which caused 2nd degree burns to right arm & face.
Employee stepped into a culvert at the surface area of the mine and twisted his left knee. Employee suffered a torn ligament and started missing time from the accident on 05/29/07.
Employee was in the process of performing a pre-shift examination, when he apparently struck his head on the roof or roof support, causing a concussion. Employee has no recollection of the incident.
Employee got his ankle caught between a track rail and a roof bolting machine and fractured his left ankle.
While shoveling crude, employee felt a pain between his shoulder blades.
Victim rolled truck on its side while returning on haul road to shovel after experiencing steering failure, incurring bruising on left side of body and head.
Individual was setting rebar with hammer, when he missed, hitting rebar and hit his left index finger with blow from hammer he was using.
Employee was climbing a ladder to get onto the dragline and the ladder kicked out from under him. He fell about three feet and landed on his right hip. He was seen at a local clinic and it was determined that he had fractured the hip. He was sent to the hospital and received a partial hip replacement. Expected to return in around 90 days.
Employee is an examiner and stated he was in the process of removing material from the travel road. He bent down and pick up a timber to move it off the road when he felt pain in his left groin area. This is something that he does all the time to keep the roadways safe to travel.
EE was replacing a bucket tooth and shank that had broken off the dragline bucket. He lifted the approx 120lb assembley several times and reported back and groin pain the next day. He was diagnosed with a hernia which would require surgery to repair. He worked until 6/26/07 and that is when he had surgery.. First day lost is 6/26/07. Estimated lost time 4-6 weeks.
Employee was standing beside roof bolter when a piece of draw rock fell striking him on the head, back and right leg.
EE was walking up entry carrying 8 belt hangers when he jammed his head into the roof.
Employee caught hand in belt on conveyor while removing stuck bag.
Employee was moving cable out of buggy road & a piece of rock fell between the mesh & rib on right side entry & struck him on the right forearm and shoulder.
The employee and co-workers were attempting to replace a motor on a stacking conveyor. As the motor was on its base it slid off and struck the employee on the left wrist.
Putting resin in hole a 3"" rock fell through the screen hitting employee on right wrist causing employee to step back. Stepping back employee lost his footing and fell on his left shoulder.
Employee was removing a pump case with a hoist, when he turned and felt something pop in his knee. He had surgery on 5/29/07 resulting in a loss time.
Raw Grind Operator was shoveling fine from rock run into the secondary crusher hopper. Operator felt a sharp pain in lower back while lifting a shovel of gypsum fines.
Member was hanging the roof bolter cable in the #5 x-cut when a piece of drawrock fell striking him on the back of his foot causing a fracture to his left foot.
After rainstorm he stepped out of loader to wash windsheild and slipped off of the fender and fell to the ground.
EE was lifting a 100,000 lb A-Frame with the 100 ton Link Belt crane using two 7,300 lb nylon chokers. one of the chokers slipped shifting the load to one choker which broke allowing top of the A-Frame to fall across the cab with the injured employee inside.
EE was starting and checking fluid levels of the haul trucks for the start of the shift. While checking truck #06-0090, EE slipped of the ladder on the front of the truck and fell backwards.
Employee was scaling loose roof; as he was attempting to move a rock, his left shoulder popped. EE went to the doctor for an X-ray, the result was negative. He continued to work and his shoulder continued to hurt. He returned to the doctor for a MRI and had a positive result on 5/8/07. EE continues to work, awaiting his surgery.
While putting up a rope bolt about 6"" from top, bolt started to bend. He tried to spin it the rest of the way up when the head of the bolt broke off & plate flew off the end of the bolt striking him in the face leaving a 2"" cut over right eye.
While on the rock breaker in the pit breaking rock, the employee broke through a large rock and in doing so the machine shook some dust off the ceiling of the excavator and it drifted down behind his safety glasses and into his eye.
No conditions contributing to accident. Tripped while stepping up to platform. Became unbalanced and fell on right leg, then both knees.
Injured employee was working with another employee taking down ventilation tubing in a fully bolted entry. When the tub was released from the roof, a piece of loose coal struck the injured employee on the top of the right foot, resulting in a fracture of his right foot. Employee was wearing steel toed metatarsal guard boots.
While descending vertical ladder to silos, lost grip on ladder and fell until arm jammed in rungs of ladder.
Employee was welding in shop on metal, flash from welder burnt both eyes.
Going to do belt splice on #6 belt; grabbed top roller underneath belt, pulled on roller & pulled muscle on right shoulder.

While tramming a continuous miner employee attempted to place a loop of cable slack onto the conveyor boom without deactivating the remote control. He accidently hit the conveyor swing pinning himself against the coal rib.
While working on the scalper cleaning out rock, a rock flipped over onto EE's hand. The middle finger was caught between the wall of the scalper and the rock.
Working on belt press, while getting down, EE jumped 2'. The cable caught him in the groin area and somehow jarred his right shoulder. He grabbed the rail with his left hand.
Employee was shoveling large rock up spoil pile and when he backed off rock became hung under blade, jerking machine.
Employee strained his back in the operator's booth while leaning over the controls of a hydraulic rock breaker.
Walking down LW face, he stepped over a double eye. Foot stuck; unable to catch himself he fell foreward striking his left shoulder and head.
Driller was stepping down from ladder while dismounting the drill. As he stepped to the ground he stepped on a large rock which rolled causing him to sprain his ankle.
Employee was using a hammer & chisel to remove right rear drive axle on roof bolter when dirt & rock fell into his right eye.
While riding mantrip out of slope, motor stalled, causing mantrip to roll out of control and hit the wall.
EE was riding outside on mantrip when brakes failed as he was toping out. Mantrip rolled back down slope, wrecking on bottom striking solid wall.
Employee jumped from moving mantrip to avoid collision with another mantrip and injured shoulder.
Picked up bucket of miner bits (20) in one hand & miner remote box in other. Hit head on low place in mine roof, twisted head, felt sharp pain in back. Pulled & strained back.
While riding mantrip out of slope, motor stalled causing mantrip to roll out of control hitting wall.
Employee was shoveling on the slope belt and stated he was walking and slipped. He stated he fell and twisted his back. He walked app. 300 ft up the slope to the surface to report the injury.
Backing miner out of right X-cut, stopped and was swinging miner over when left tram lever on remote stuck, causing miner to swing downhill. The auger of the miner rolled over EE's leg, causing a break.
Employee was removing a rock from roadway when he felt pain in his lower back.
While walking out of building (lunchroom), sprained left ankle on a cement ledge.
EE was operating the roof bolter, when a piece of rock fell from the top and hit the canopy. This made the EE make a quick and sharp movement that caused a pulled muscle in his lower back.
Employee ""ALLEGEDLY"" injured his neck and shoulder when belt was being changed out. Belt structure came apart and fell on him while sitting in scoop. He finished the shift.
After a repair on the dragline, the employee complained of pain in his shoulder. He was examined by a doctor, and diagnosed with a strain. He was allowed to return to normal work duties. On May 23, 2007, he complained of more pain, and returned to the doctor. At that time he was excused from work pending further medical tests.
Employee stepped out of the cab of a truck and rolled his ankle.
Employee was installing a rib bolt, when rib fell and struck employee on the left shoulder.
Lifting a rock up off of miner, rock broke off causing him to fall backward onto the ground.
As employee was driving a Euclid R170 end dump up an incline, he noticed an engine noise, so he stopped the truck and shut off the engine. The truck began to roll back a distance of 75 yards before he turned the truck into the berm, causing it to roll onto the truck's cab side. The impact caused a window to shatter. Glass caused a laceration to the right side of his face and eye.
While storing a soldering machine, the machine slipped and traumatized the left hand.
Walking to section from manbus. Left foot stepped on uneven bottom causing ankle to turn. When this happened, fell face first on bottom, striking right knee. Resulted in lost time as of 6/19/07.
Employee was helping to assemble framework for a new conveyor. He was attempting to align a bolt into it's corresponding hole, he got his finger caught between the threads of the bolt and the framework.
While reaching for drill steel, a piece of material struck his left hand pinching his left index finger between the material and the steel.
EE was in the process of installing a roof bolt. He drilled a hole, inserted the resin and bolt into the hole. Due to mining height, the bolt had been bent prior to inserting it in the hole. He inserted the top part of the bolt in the hole and as he pushed on the bolt to straighten it out, He strained his back.
Caught left foot between scoop bucket & p/c.
The employee was reaching for a powerpack as a co-worker handed it down to him. He claims he pulled his right shoulder when the other employee dropped the powerpack to him. He reported it, but thought it would be OK. However, the pain has persisted. An MRI on 8/13/07 showed he has had some trauma in the shoulder and he was given 2 shots.
INJURED HAD LIFTED A TOOL BOX LID UP TO GET ROOF DRILL BITS. THE LID FELL, CATCHING INJURED'S RIGHT HAND BETWEEN THE LID AND THE BOX.
Right side bolter operator was attempting set an extra bolt along the right rib and the bolter drillhead would not reach to that location, the left side bolter operator attempted to move the bolting machine so it would reach and it jumped pinning the right leg (kneeling position) against the floor, severly bruising the top of his right leg muscle
While working in the feed end employee misstepped on the grating of #13 screw and twisted his left ankle.
Injured was on a ladder building wall when a highline cable fell and got caught on motor and knocked over ladder injuring his lower left arm.
EE was filling hydraulic tank on his loader. After filling tank he dropped the 1 gal plastic jug to ground, wiped oil off his hands on rag and proceeded down the steps on loader. His hands slipped off the handrail and he fell backwards hitting his head, shoulders and back on the ground.
While employee was trying to get a rock out of the belt chute he slipped and fell on the other beltline causing contusions to side.
Member was changing a scoop tire when the tire fell and hit him in the chest and arm. Member continued to work until May 4, 2007 at which time he was examined by a chiropractor who stated that he suspected some disc damage and has him taken off work until the diagnosis is continued; a MRI is scheduled.
Crew was setting posts in preparation for new bleeder system. Employee said sometime during the night he pulled something in his neck or back. He finished the shift and reported it to the shift foreman. He has not worked since 4/23/07.
The employee was climbing from the feed end toward the discharge end of the unit #224 course refuse screen to check the oil levels. His foot slipped and he fell nearly seven feet onto the plant floor below. He was taken by ambulance to the ER where he was treated and released. He started missing work on Saturday, April 28, 2007.
Was floating out roof bolt operator. While adding steel together to drill deeper hole, steel cut finger on left hand.

EE was in the process of entering operators compartment of a skid steer loader, when he turned and felt pain in his lower left back.
Employee walking down panline, stepped over a relay bar and foot hit wrong and he twisted his left knee. EE did not start losing time until 5/10/07 due to surgery.
EE came into the office on 4/24/07 to fill out a incident report for a sore knee. He indicated that while coming down the staircase at the #1 Crusher he slipped on the stairs about 4 to 5 steps from the top and was able to catch himself with the handrails. He did not fall but said he twisted his left knee and did not need to go to the clinic.
Employee was walking on top of rebar mat when his foot slipped into a hole in the rebar mat, causing pain to his foot & back. Did not seek medical attention until 5-1-07. Called him 5-9-07. His care is now beyond first aid.
While climbing the stairs carrying a return idler weighing approximatly 40 lbs, employee felt a pop in his left knee followed by pain shortly thereafter.
Employee was greasing and tripped and fell face first to the ground catching himself with his hands. Employee complaining of lower back pain.
Employee was stacking belt structure when a piece slipped out of hand and struck his left knee.
EE bumped his head while working on a haul truck. He didn`t have any problems at the time but awoke the next day with pain in his arm. He has a diagnosis of degenerative disc disease and bone spurs.
Employee was changing a halogen fixture on the 33 shovel, and as he pulled the fixture out of the housing, the broken bulb filaments made contact with the frame he was holding, causing electric shock and burn to a finger on his right hand. He was taken to the ER, admitted for observation. Employee had failed to unplug the light prior to starting work.
Employee was walking from the F-9 bleeder inlet when his left foot caught on a rock causing him to twist his left ankle. The employee continued to work until 8-11-07 when he began missing work due to the injury.
Employee was shoveling off spill material by 7 foot crusher and sustained lower back pain.
Truck Driver climbed light tower to rotate lights. Stood on bracket and turned lamp assembly not entire mast. Mast slide down jamming right hand between tubing and bracket; used left shoulder to lift mount and pull hand out. Transported to hospital, injuries: badly bruised right hand, abrasion on left side of neck from the mast cable, contusion of the left shoulder.
He was walking on a belt on the ground that was covered in coal dust & slipped twisting his right knee.
A pumper was attempting to open a drain/relief valve on a dewatering pump when hot water in the pump, caused by low suction, sprayed his left hand and the left side of his face.
Mechanic was installing hydraulic cylinder onto excavator. He removed pin to re-align. The other end of cylinder was suspended by rigging; the other end of cylinder slipped out of the pin socket, swung away suspended then swung back and struck mechanic on the jaw causing fracture to jaw. The mechanic stepped down from ladder and asked for assistance.
HE WAS MOVING PINNER AROUND SUPPLIES AND STEPPED IN A HOLE AND FELT KNEE POP. ** STARTED MISSING WORK ON 05/25/07**
Employee was installing gate on mine road and while lifting on gate became unbalanced. Employee originally starting missing on 4/21/2007 for personal illness. Employee notifed employer on 6/25/2007 he was going to seek medical treatment for back strain which occured on 4/20/2007.
Operating loader, extending back to reach levers and steering, and noticed a sharp pain in his lower back. He missed 11 days of work and was released to come back 5/8/07 with 30 lbs. Lifting restriction. He had elective non-work related surgery on 5/8/07 and did not return to work until 5/15/07. He was released from lifting restrictions on 5/18/07.
Person was making exam of permissible pump, stepped on 3"" plastic pipe with right foot while walking and slipped.
EE was using a snap-on filter wrench with a 24"" breaker bar to loosen a filter the breaker bar slipped out of the filter wrench socket and his left elbow struck a strut housing. No reportable medical treatment until 7/12/07.
The injured employee felt pain in his lower groin area as he was stacking up a solid block stopping. The employee continued to work until 5/12/07, at which time, he was diagonised with a hernia and will require surgery to repair.
Employee was kneeling on CBI, throwing rocks off. He lost balance and fell landing feet first.
Emp was welding and a piece of steel fell off the lip onto left foot.
Hanging CM cable, while miner was being moved he felt pain in ankle after stepping on a rock. He twisted his ankle to the outside.

Employee was standing in front of miner cable, miner pulled forward and rt. foot got tangled up in cable.
Employee was replacing liner plate in feeder chute. Liner shifted catching his left thumb. Diagnosed with a fracture. (Started missing time 4/26/07.)
Getting off loaded supply car. Foot slipped on bundle of roof bolts, causing to fall to ground.
Employee stated he was installing a roof bolt when the bolt bent. He put his arm up in a defensive posture and the bolt struck his left forearm. This resulted in a fracture.
While plastering, employee fell off ladder and landed on his back on top of the plaster bucket.
Employee while driving company van, dropped cell phone, leaned over to pick up phone and van struck berm on side of road. Employee's head hit steering wheel, causing laceration to nose, requiring stitches.
Stepping down off of the 928 loader. Landed wrong on the ground and broke bone in his foot.

Employee attempted to adjust position of cylinder with sledge hammer. When cylinder aligned the air pressure caused it to move toward the employee about 4"". Cylinder hit hammer driving the handle into his chest.
While carrying stemming (rocks) in a 5 gallon bucket he stumbled on a rough terrain, lost footing, pulled groin.
EE misjudged distance from last step on loader to the ground.
Employee was pulling on miner cable and pulled muscle in groin.
EE was hammering a nail into a board next to the mine roof to install a line curtain in #10 entry. While installing the nail, a piece of a petrified stump fell from the mine roof striking him on the head, left shoulder and left leg. The incident caused abrasions.
EE and Another ee were at the face. He was having trouble with the miner remote, while they were looking at it, a piece of rock lit his hat. It was 1'x1'x2"". The rock fell between roof bolts. Later in the shift he said his neck was hurting.
EMPLOYEE REPORTED: HE WAS HAVING PAIN IN SHOULDER DUE TO INSTALLING CRANE CABLES. EMPLOYEE WAS SENT FOR MYELOGRAM AND DIAGNOISED WITH DEGENERATIVE CHANGES AND BULGE ON 6/22/07.
Employee was removing fish plates from track. He was using a 3/4"" drive wrench. He stood on the wrench to try to break the nut loose. He fell and landed on track twisting right ankle.
Pulling on hoses while working on longwall setup, he strained his lower back.
Employee was center bolting in the 7 left 5 south section when a 5 foot section of auger stuck in the hole he was drilling. He started to install the second steel and the stuck auger fell from the hole pinching finger between the steels.
Employee was standing on a berm watching Haul truck training. Employee went to get off berm and twisted her right ankle in the process.
Employee was attempting to remove a piece of rock from cable reel of shuttle car. The employee slipped and rolled his ankle. Result of the injury was a fractured ankle.
Employee was inside transition and went to get on ladder to get below to recoup duct. He swung leg over to get on ladder rung but missed it. Having thought his foot was close to rung he let his weight go to make contact with it. Since nothing was there he skidded down ladder a way and then fell off of it.
EE was bolting on the offside. After drilling the hole, he installed a 5/8 x 6' rebar with resin 2x6x12 board & 6x6 plate. As the bolt was being shoved into the hole, it caught on something and the bolt plate struck EE's right wrist. The bolter was not rotating.
Employee's foot slipped while climbing onto FEL causing pain to rightt shoulder; employee is scheduled for surgery on 06/28/07.

Railcar Loading Operator was releasing the brakes without using the brake release lever. The Operator was loosening the brakes by turning the brake wheel counter clockwise. The Operator pulled a groin muscle while trying to loosen the brakes on the railcar.
The employee was changing a hose for the water discharge sump. At the completion of his task he hopped or jumped down from on top of the continuous miner, landing on his right foot resulting in a fractue of the 5th metatarsal bone.
Employee was driving a forklift down the hill on N side of Met lab & either hit the brakes or reverse pedal at the top of the hill, lost control, and started up the bank to the left. Forklift laid over on its side. Employee sustained a lacerated elbow, which required sutures, road rash on back of shoulders and a fractured nose. He claimed the engine stalled.
Employee was assigned to wash the T541 haul truck. After letting the bed down, he was descending the boarding when his foot got caught in his raincoat and last rung of the boarding ladder, causing him to fall to the ground injuring his left knee.
Employee was removing a block curtain when a shuttle car turned into the x-cut hitting his left foot with the shuttle car tire causing ligament damage.
EE was pulling shields using infared system. He was bent over looking at #161 shield to see why it didn't pull all the way in. Employee was helping crane men unlock strongback on floor when he noticed a anode was not properly in the saddle and decided to get on rack to fix anode and doing so he fell off rack.

On 4-17-2007 Miner was cleaning out under a conveyor tailwheel with a shovel and a rake. Miner was pulling out material wth a rake and felt a ""pull"" in his back. He attempted to give ""pull"" a chance to heal. On 05-02-2007 Miner requested a Dr. appointment. This resulted in a diagnosis of an acute lumbosacral strain requiring physical therapy.
EE was torquing down the lug nuts on a truck when torque wrench ratchet slipped. Wrench came down on leg & broke leg/ankle.
Employee was repositioning a 50lb bag of sand when he stepped on a pallet lift. The lift had a bolt break. The lift fell on the employee's foot. He went to doctor on 5-7-07. Doctor said looks like swelling; come back if anymore problems. Emp. went back during Sept. Doctor said surgery would take care of cyst on 9-10-07.

EE lifted a 5 gallon bucket that had bolts in it. (about half full). Felt pain in his lower back.

Wireman was installing a control transformer in motor starter compartment using a hand threading tool to tap threads into two existing holes in the back wall of the compartment. The tap's pointed end penetrated the wiring which ran along the back side of the compartment metal wall thereby causing the short circut from the energized wire to the ground via the thread tap.
Employee made a mis-step while throwing the miner cable against the rib; he felt pain in his lower back.

The main air header isolation valve was closed and the pressure relief drain valve was open. When the operator opened the quick disconnect coupling the pressure in the header popped open the header and dust was blown all around and got dust in operator's right eye.
After employee finished fueling D7 Dozer and was stepping down he lost his balance and fell approx. 2-3 ft. to the ground landing on his left wrist and shoulder resulting in a left wrist fracture.
Technician was straightening fender on d-11 using 20 lb sledge hammer. As technicim was making a downward blow w/hammer. the head struck the suppressiom system on the d-11 caught the handle to come toward the technicion. The end of the handle struck the left inner thigh area between the groin and knee cap, resulting in severe contusion.
While performing routine maintenance employee slipped on a piece of steel and hit chest against steel beam.
Employee finished fueling excavator and was climbing off when he slipped and fell off machine dislocating his left ankle. Taken off work 4/15/07.
While attempting to force a gear shifter forward in order to dump a haul truck, EE felt a pop in his right elbow.
EE was injured while handling material performing roof bolter duties. During the drilling process of bolt installation He drilled approx. 6 ft deep hole & attmpted adding a middle section of drill steel when the all ready inserted top section of drill steel fell striking him on the left hand. Impact from the drill steel caused fracture to the left hand and little finger.
Hurt both shoulders on 04-13-07 pulling miner cable. Reinjured on 04-18-2007 when stepping off man bus. Did not report accident until 04-18-2007, left work and went to Dr.
Employee was found on ground by another scoop man while changing batteries on scoop. Told him that he was shoveling out battery tray and back went out. Told foreman same thing.
While loading the oil drum, it slipped and when he tried to stop it, the drum caught his left ring finger against the truck.

An employee was stepping off of a gearbox after adding oil when he lost his balance and fell onto the catwalk on his shoulder.
EE was riding a 3 wheel bicycle when his left rear wheel caught a 4 inch hose causing the bike to come to a sudden stop. The forward motion caused the EE to exert more pressure on his bent left elbow. EE heard a ""pop"" and felt a sharp pain in his left elbow. Diagnosis is a torn muscle in his upper left arm. Surgery is scheduled for April 26, 2007
Employee stated he was using a bolter, installing chain hangers in a new cut out drive area. The top was higher than normal and he was using a longer steel to drill the hole, when it hit a crack in the strata, jumped up, started to wobble. He reached out to grab the steel and also hit the up pressure at the same time and the steel bowed and struck the back of his hand.
EE was running shuttle car, ran shuttle car into rib, jarring neck into canopy.
The Employee while exiting the vehicle lost his footing thus holding on the support bar felt a sudden pain in his right arm.
Employee was helping to lift a pipe and he lost his grip on it and a metal ring on the pipe hit his knee. He was seen at the ER, placed in a knee immobilizer and released to restricted duty.
Was pushing brush-blade caught, dozer overturned.
At 1300 incline station, employee was changing rollers from LHD bucket. LHD operator prepared to dismount and install chocks. LHD rolled several feet striking a Kubota which struck the employee. Taken to doctor initially, doctor found only bruising. After employees normal 5 days off, went to another doctor who gave restricted duty. Restricted duty started 4/20/07
Ee was welding on a pipe. When he bent over, he felt something pop in his back. Ended up being muscle strain.
Loader operator tried to pull a 5 lb. rock out from behind his bucket. The rock didn't move but he felt something pull in his groin.
Twisted back getting off of machine at quitting time.
Alleged injury occurred when he was striking a pick hammer in order to drive a staple lock that was frozen out of a hydraulic hose. A fleck of metal came off either the pick hammer or hammer being used to strike the pick hammer and entered the left hand. Note: Taken to Dr. same day, fleck removed 4/16/07, no work missed.
Working on opening a chute door to do repairs on inside. The door wedged and when it came open, it caught his hand.

The ee was assisting in loading of the high voltage cable into the high voltage car when he stepped into a hole causing him to fall. The high voltage car ran upon his right leg resulting in contusions to his right lower leg. The employee continued to work until 4/18/07 at which point he was taken off work by a physician.
Injured was cleaning in the jackline and putting a piece of rock under the tailside skid. He was pushing rock further back under the skid when the positive set activated and caught his finger between the rock and the bottom of the skid.
Operator was letting scoop bucket down. Injured saw a block underneath and tried to get block out and pinched thumb under bucket.
Two employees were lifting a rock approx. 100 lbs off the pusher table when one tripped over a rock on the ground, fell and the rock fell on his leg.

Injured EE was tightening bolts on upper section of vertical roller. He was working on a ladder in an awkward position holding the nut with his lt. hand and working a ratchet with his rt. hand. EE leaned over and around conveyor bedframe and roller mount. He felt a ripping sensation in his left shoulder.
While unloading high voltage cable from a skid on a 25x the alledged injury occurred when the 25x slide into a roadway pot hole jerking the high voltage cable.
Mechanic was changing water sprays on the scrubber motor and a rib (12x4x8) fell suddenly, struck & pushed EE against Miner. The rib fell in a way that the Cont. Miner took the blunt of the weight & force. The EE was not pinned & was able to walk back to the back of miner. The EE was conscious & talking, but pain in shoulder & chest.
The employee was helping install belt conveyor structure after a belt move. He was helping lift belt structure in place when he strained his back.

When EE climbed the loader ladder to enter his cab, he missed the handrail, slipped and fell to the ground onto his right hand. His right wrist broke as his hand struck the ground.

Employee got on the passenger side of 785 rock truck to clean mirror. Said he tripped over CB coax cable, fell and hurt his back. Employee finished shift, drove himself home. Tried to work the next day but said his back was hurting too bad.
Employee was operating a wheel scraper removing interburden from above the lower coal seam. It was snowing hard and a depression in the roadway was not readily observable. A scraper tire impacted the depression resulting in pain in the employees back. He was unable to return for his next shift.

As the employee was attempting to re-position the drill, his right index finger was caught between the drill steel and the chuck of the drill, resulting in a laceration requiring 7 sutures to close.
Employee states while lifting bag of mud, he experienced pain in lower abdominal area. Did not desire treatment at the time. Symptoms did not improve.

While air arcing flange, it shifted approx. 12"" and employee stepped backwards. At that time his foot went off the scaffolding and under the midrail. The employee then topped over the midrail and fell to the floor 8' down. Employee broke his left femur.

Rock broke loose and fell from highwall. Employee was working on drill; rocks from fall pinned him under the deck of the 5026 Drill.

EE was feeding the crusher with the excavator. He dumped a load of rock in the feeder. It appears a rock hit the chains in the crusher displacing them allowing another smaller rock to exit the crusher hitting ee in the forehead, while he was in the excavator.
Employee was climbing a 4' ladder. As he applied his weight to the 2nd step, the ladder turned. He fell backwards & landed on his back & shoulder. EE continued working. Did not miss any work. On July 24th, employee complained of back pain not going away. Took him to doctor. X-ray showed fracture at T12-L1. EE is off work now.

EE had raised the hood on an ex370 excavator during his pre-shift. While the hood was in the raised position it unexpectedly fell, striking him, causing him to lose his balance and fall approx. 7 feet to the ground. EE landed on his right arm causing a fracture to the elbow.
While cutting electrical splice employee cut two left fingers.
Employee opened the hatch door, failing to latch the door open and standing partially through the hatch, it swung closed, hitting his head. Employee was not wearing his hard hat, did not mention any extreme pain to the plant operator whom he spoke to at the time of the accident. doctor diagnose concussion.
Employee was removing the belly pan when it fell striking his leg. Took employee to doctor and released him to go back to work. Infection got into the leg and employee had to have the leg lanced and the infection cleaned out. He had the surgery on 04/26/07.

Contractor was placing piece of stone into stone splitter. While lowering top jaw of machine, the stone fell forward. The Contractor tried to stop stone from falling out of position. The splitter teeth then contacted his fingers. When he felt pressure on his fingers, he jerked his hand back, pulling the skin off the tips of two fingers. The two fingertips required sutures.
Employee was using an impact wrench to remove nuts from the ball mill bolts. He was bent over and when he reached to his left he felt his right knee pop. Doctor felt it was just a strain on first visit. It was later diagnosed as a torn meniscus. Employee had his knee scoped on 7/9/07 for repair. First shift missed was 7/9.
Sitting on ground, reached up with left arm to pull self up, lost balance fell with full weight on left shoulder. Completely dislocated left shoulder.

A brace was being hoisted by a crane. The connector was climbing into position. The flagman for the crane was not properly positioned and did not wait for the connector to get into position. The flagman brough the brace in too low and struck the connector in the back pinching him into a beam. He fell and his safety equipment caught him.
EE lifted a tool bag that weighed approx 60lbs. He felt a pull in his lower back.

Then EE used both hands to lift bucket. A day later, employee felt pain in shoulder. **Employee returned to regular duty. On May 22, he was unable to work. Dr determined it to be result of 4/22/07 incident.**
The employee was tightening bolts on the hoist and fell backward falling to the ground hitting his head on a work bench.
Corner of rib fell from behind rib screen in 10 west longwall #1 entry. EE's hand was caught betweend rib screen & stageloader. Surgery to repair tendon in hand & x-rays later showed broken wrist.
Miner was standing on bed of flatbed truck unbinding a detached backhoe boom and stick section. The loosening of one of the chain binders caused the load to shift and roll hitting the miner and knocking him off the truck resulting in cracked rib and knee injury.
