A roof fall occurred in the #11 seam #7 entry in the pillarline inby cross-cut #34 and extending through the pillarline between cross-cut #35 and cross-cut #36 measuring approximately 115ft. x 18.5ft. x 4.5ft - 5ft. The area was supported by 30 inch fully grouted roof bolts. A slip was determined to be the cause of the fall.

Wire for emergency stop button broke in two.
In #1 entry of C-1 Mains. Miner was taking a side cut to sratr # 8 Left Room. The roof started working in the Intersection of #1 Entry. The miner was backing out. When roof fell in pulling bolts inarea. Approximately 17' W x 16' L and approx 10' in height.
A roof fall occurred on the 3 Left, 4 West panel at crosscut 34 in entry 2 and the crosscut from 2-3. The area had been cribbed and timbered prior to the fall due to adverse roof conditions observed in the area. The fall is located in the returns for the 005 section.
An ignition occurred at 82+14 on the F-14 Longwall face at #28 shield. The ignition lasted approximately 2 seconds and was extinguished by the water sprays and a wash down hose from the sheild. There was no damage to equipment and there were no injuries.

Elevator at Crabapple continued to operate properly, until 3:05 AM, when an electrical malfunction in the solid state drive circuit board caused the elevator to stop. No one on the elevator.

Unintentional roof fall occurred, 25' in length, 12'-15' in width, 4' thick, in belt entry 3400' outby working section. Was bolted with 4' bolts. Cleaned area up & rebolted with 6' point anchor bolts.
A roof fall was found during an aircourse examination by an MSHA inspector and a Safety Technician. The fall was located in a worked out panel located in the 2 left, 5 West. The fall was located adjacent to the examiners route of travel.
Unintentional roof fall 20 ft. long, 20 ft. wide, 5 ft. high in No. 3 Return, No. 1 Entry, Crosscut No. 49. This is a continuation of an old fall in adjacent entry.
The grout in an unmapped borehole pushed out releasing a low oxygen condition into face of the # 7 entry of the Sioux working section. More air was introduced, Low O2 cleared under supervision of Federal Inspector and Section Foreman. Borehole was securely plugged from bottom.
Smoldering belt shavings on 10 H belt, occurred at 6 am on 2/27/07 was determined by MSHA investigation on march 13, 2007 of an incident that they determined that it was a reportable fire under part 50. However the operator still believes the smoldering area once found was extinguished in less than 10 min. is not a reported fire. This form is being sub. (#7024738)
A bounce with floor heave occurred at the head gate of the longwall moving stage loader and controller, impeding travel from the longwall face.
Crabapple elevator was operated 35-40 times without incident & once again it stopped. No one on the elevator.
An unplanned roof fall occurred in the Winifrede mine that measured approximately 8' in height, 7' in width and 19' in length.
Roof Fall on mains section #4 entry 1 blk inby station 1272. Fall trapped fletcher roof bolting machine in #4 hdg at face. Fall above oncorage no persons present at time of fall. Poor grology in area.
Crabapple elevator hoist was rendered inoperative due to an electrical malfunction in that a solid state drive circuit board shorted out. 22 people on the elevator when it stopped. No injuries.
While lowering a roof bolter in the supply slope, it became fouled along roof and rib. Hoist operator tried to pull trip back out but chain broke and allowed bolter to slide off of material car. Supply hoist was out of service for more than 30 minutes.
An ignition occurred at 82+25 on the F-14 longwall face at #28 shield. The ignition lasted approximately 10 to 15 seconds and was extinguished by the water sprays and a wash down hose from the shield. There was no damage to equipment and there were no injuries.
A roof fall occurred at 1st North Main East, Crosscut 72, Entry 6. The fall measured approximately 20' long x 16' wide x greater than 9' thick. The fall did not result in any damage to equipment or property.
Roof fall found, during a supplemental exam. #3entry xc28 outby area. Fall was 19'Lx30'Wx10'Thick, Fall was in belt neutral, and will not be reclaimed, fall was cribbed and dangered off.
On 003 Section while pillaring, test holes checked by foreman, an unscheduled slip occurred that unintentionally fell and covered miner & shuttle car. No injuries. Equipment unrecoverable.
Elevator was down due to malfunctioning in the limit switch.
A roof fall was found in the 8 west bleeder between the #1 and #2 entry at x-cut 44. The fall measured 40' long x 18' wide and 6 1/2 to 7' high.
Elevator on surface with doors opened. Could not get doors to close. Found door closure motor bad. Replaced motor; elevator operable again 3:05 pm.

Fall occurred die tothe detrioration of the immediate roof around existing bolts. The area was supported with 8' expander shell roof bolts w/2' resin anchor. Area was mine in Oct 1993. Cribs were set along clearance side cross-cut.
Elevator was out of service when the auxiliary contactor on the M.C. malfunctioned.
A roofall measuring 20'L x 17'W x 6'H occurred in the #5 heading 40' inby spad No. 8984 of 003 MMU (002 Section). Area was bolted with 5' glue bolts.
Coy was making a weekly examination and found a roof fall in the Big Block submains. the fall is in the #2 entry Between break 15 and 16. It is approx. 20ft.X 50ftX 8ft thick.

A roof fall was found at entry 0 41+30 6LMS by examiner making primary escapeway. Fall was in 4 way intersection that had been supported with 8' full resin bolts. No injury or property damage resulted. Fall will not be cleaned up.
A roof fall occurred in the #10 entry back cut measuring 30' long X 17.5' wide X 8' high. The face had a 12' cut unbolted. When it fell it caused the intersection to fall also. The intersection had 5/8 x 60"" bolts. The area was cribbed off and permanently dangered off.
A roof fall was discovered at 9 left 2 east. The fall was approx 8'h x 20'w x undetermined length. The fall will not be cleaned up. It will be breakered off.

We had a rock fall in an intersection in the intake one break outby 5305 spad. The rock fall is approx. 30' long, 20' wide and about 18' high. Fall was found on weekly examination. It is in 6 entry. There is another travelable intake. It is not affecting the ventilation at the mine
A roof fall has occurred on 1-South, 19th crosscut in the no. 7 entry intersection extending toward the no. 6 entry. The fall was 40' x 18' x 5-6 feet in thickness. The area was originally supported with 5' High Tech Bolts. The fall did not affect ventilation.
Unintentional roof fall occurred at x-cut #13 in entry #3 at face area. Fall covered up the Lee-Norse 265 miner. No injuries to any employees.
While conducting the bi-weekly inspection and operation the hosit was unable to decend down the shaft due to a build up of ice in the shaft. The build up of ice covered approx. two-thirds the diameter of the shaft. The mine was immediately evacuated and recovery plans made to return the mine to normal operation.
A roof fall was discovered by the mine examiner during his regular routine. The roof fall was in the #2 entry of the 1 NW Mains, approx. 60 feet inby spad no. 3020. The roof fall measured 20' wide by 20' long by 4' thick. It does not effect the mine ventilation. The area was barricaded to prevent persons from traveling in the area.
A roof fall was discovered in the 2 North 2 mains at 89 crosscut in #1 entry. The fall measures approximately 30 feet long, 18 feet wide and 7 feet thick. The area is supported with 5 foot fully grouted resin bolts and 6 x 6 timbers.

A roof fall occurred on the 1st Main North belt at 37 1/2 crosscut (#3 Entry). Fall was 12' long, 12' wide and 6 ' high. Area was mined in Feb 1989. Fall occurred due to the deterioration of the immediate roff around existing roof bolts. Area was supported with 8' long expander shell bolts with a 2' resin anchor. Mining near a fault line, the intersection of #3 Entry fell approximately 10 to 12 feet high. There was no damage to ventilation or equipment on the section. No miners were involved in the fall, so therefore no injuries.
A roof fall occurred on the 3 Left, 4 West panel in entry #4, between crosscuts 35 and 37. The fall fell on the 5A belt halting production on MMU-005. The cause of the fall was weak laminated shale. The fall was cleaned up and the belt was returned to service.
The 1085 hoist was inoperable for approximately 2.5 hours when one of the skip tires went flat. The tire was repaired and the hoist returned to operation. Note: The second skip was operational during this period and the hoist was still functional as secondary escapeway. There was no damage to the hoist.
At approximately 8:15pm, the elevator at the Galatia North Portal went down when the car dogs set on the way down, approximately 10' below the surface. The switch and actuators were checked and found to be functioning properly. The elevator was put back into service at approximately 9:00pm.
Our weekly examiners found a roof fall in a worked out area of the 5 Left, 1 Left, 4 Main North A worked out panel. The fall was located in entry #3 at crosscut 33. The fall was adjacent to route of travel of the examiners.
An unplanned roof fall occurred in the #4 entry of Second Left inby survey station #403 1 crosscut. Fall was approximately 19' wide, 20' long and 4' plus high. Area was bolted with 4' tension re-bar bolts per the approved Roof Control Plan. Area was timbered and dangered off. The fall changed the normal route of travel. Did not affect any ventilation.
A shuttle car operator was spelling the regular bolter operator through lunch. He tightened a conventional pin (in the rib), and noticed that the friction created by the wrench/bolt head ignited methane. Water immediately extinguished the flame.
Pin loosened from auger head causing the head to stay approximately 50' underground in pit area #43.
Encountered fall of roof at Station 10+50, Entry #3 (Haul Road), on the Main Northwest. The fall measures 17.5' x 50' x 12' thick. Secondary escapeway has been routed to Entry #4.
During the night shift the shift foreman found a roof fall at #31 xc in 1st Rt, belt line. 20' wide, 15' high and 30' long.
A roof fall was discovered at Break 70 on 3-North in the #9 entry. The fall was 20' x 60' x 5' in thickness. The fall extended through the break to the #8 entry. The area was originally supported with 5' High Tech Bolts with 8' Super Bolts as supplemental support.

The Miracle Run elevator would not return to surface landing from Miracle Run bottom. The Schindler repairman determined that the brushes and bearings needed to be replaced in the hoist motor.

An unintentional roof fall occurred at Crosscut 22 in the Galatia North. The fall occurred in the crosscut between the belt and travelway. The fall was approx 60' in length, approx 40' in width and approx 8' thick. The fall did not affect ventilation or passage of miners. The fall will be cleaned up.
A roof fall occurred in the bleeder entry behind the longwall face. The roof fall was not above the anchorage zone on the installed roof bolts, however ventilation controls were damaged, disrupting ventilation.

The 1700 Sump electrical station went down due to moisture on the contacts that was a result of a previous power outage that occurred off site. To re-energize the 1700 sump electrical station, the hoist was intentially shut down to allow the electrical system to load share so as not to overload the system. There was no damage to the hoist.
Several power outages occurred on day shift due to severe snow storm in area. These continued into the evening shift. Due to continued problems with restarting main fan and hoisting facilities, the evening shift was cancelled and mine evacuated. The midnight shift was also cancelled.
A roof fall was detected above the anchorage point on 22-M longwall section. The fall was in the belt entry in front of #1 & #2 shield, along the solid side. 7'L x 10'W x 12'H.

Sunday Night 2-12-07, 3rd Shift found a roof fall during the preshift exam in 2 West, outby section, #3 entry belt neutral intersection, xc19, was bolted with 6' resin bolts, mine had been idled and fall was cribbed and dangered off. Fall was 30'x30'x8'hi
HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 02/12/2007 at 09:45 am, the Cowling Portal Elevator was taken out of service for preventative maintenance to shorten the rope one inch. The elevator was back in service on Monday, 02/12/2007 @ 11:20 am.
Elevator down. The Govenor Rope has some rusty places where it rests on the shive wheel at the bottom. National Elevator found broken strands. Rope is ordered. National Elevator will replace ASAP.
On 2/11/07, an unintentional roof fall occurred on the Main in #3 entry at cross cut #5. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 20' long x 20' wide x 10-15' high. The roof fall will be cleaned up.
The East finley elevator was down for more than 30 minutes due to the control cable caught on a conduit hanger pulling the cable out of the junction box. There was no entrapment.
Msha issued neg findings 6-1, event 4260825. Then on 6/18 issued #2 on same event, that there was an ignition concerning scsr. Cont.miner punctured while loading gob onto a scar & caught fire & extinguished within 5-7 min. This is not considered an ignition & disagree with Msha's 2nd paper after closing original out on 6-1. Being reported only to abate 7284820 citation 6/18.
Denny & Sons (L379) was in the process of unloading his truck when the weight shifted because of the coal being frozen to the bed of the trailer. As the weight shifted it caused the trailer to give and the result was the trailer falling onto its left side. The truck was sitting in a level area when the trailer shifted. No injuries as a result of this accident.
On 02/09/07, an unintentional roof fall occurred on the 1st South Panel off the 2nd West Sub-Main off the 3rd South in the #5 entry at cross cut #29. This roof was supported with 10' point anchor roof bolts. The roof fall measured 30' long x 18' wide x 10' - 12' high. The roof fall will not be cleaned up. This fall is in a worked out area.
A roof fall occurred in the bleeder system on a pillar section. The fall does not impede the ventilation system.
Unintentional roof fall discovered at above date and time. Fall measured approximately 4'H X 8'L X 12'W. Supported with 5' torque tension bolts. Fall located 1 block inby station 5100, #6 entry. Fall caused a hazardous condition in the intake escapeway and inby section workers were withdrawn.
Icing had occurred in the accumulator air tank and air lines for brake mechanism due to extremely cold weather and moisture content in the system. Problem occurred at shift change; UG persons evacuated via Production Shaft.
On the morning of February 8th, 2007 a small rock burst occurred on the 3700 level in the hanging wall, about halfway between the 230 raise and the 244 raise at track level. The outburst disrupted about a 40' length of hanging wall. It also knocked down & disrupted compressed air & water lines. This interupted mining in the 244 & 291 stopes. No one injured or ventilation impaired.
A spark from the miner bits ignited a bleeder of methane gas near the footwall. The flame was described as a flash that was put out with rock dust.
Jerry Minor Trucking (BA7) was in the process of unloading his loaded truck when the weight shifted because of the coal being frozen to the bed of the trailer. As the weight shifted it caused the trailer to give and the result was trailer falling onto its right side. The truck was sitting in a level area when the trailer shifted. No injuries as a result of this accident.
An unintentional roof fall was discovered on Grassy Creek Mains, # 7 entry Brk. 81 at S.S. # 14680. The fall measured approximately 20 ft wide X 64 ft long and 10 ft high. This fall will not be cleaned up. No equipment was involved, ventilation was not impaired and there were no injuries.
A roof fall occurred at Crosscut 55-56, Entry #5, 1st North Main East travel road. The fall measured approximately 20' long x 16' wide x 12' thick. The fall did not result in any damage to equipment or property.
#4 Entry Roof Fall just inby 5867 Station. A shuttle car was caught x-cut 4-3 and 4 man Personel Carrier inby in #4.
The Cedar Creek Service Elevator was out of service from 10:00 pm on 2-7-07 to 1:00 am on 2-8-07 due to a bad elec. contactor. Repair service was called out & placed the elevator back in service at 1:00 am, 2-8-07.
A roof roof fall was found in the bleeder while conducting a weekly examination. The fall did not affect ventilation. The fall was approximately 110 ft. by 20 ft. by 7 ft. thick. The area was dangered off and roof support was added to the area to prevent another fall in the area.

A roof fall occurred on 2/7/07 at 7:55 AM at survey station 2 + 80 entry 1 5rms unit. Fall was in a four way intersection that had been mined March of 2005. Area had been bolted with 6 & 8' full resin pins. No injury occurred but 300 feet of cable was lost. Area around the fall was dangered off adn resupported. There are no plans to clean up the fall.
A roof fall occurred on the 3 left 4 West panel in entry #1 between crosscuts 34 and 37. The fall was found when we were cleaning a new roadway around a previous fall.

A roof fall was discovered in the 2 North #6 mains, 68 crosscut, from #1 to #2 entry. The fall measures approximately 70' long, 18' wide and 6' thick. The area is supported with 5' fully grouted resin bolts.
A fire started in the production plant near the wood stove. The damage that ocurred was to the roof, plumbing, multiple electric circuits, rail cranes and the building interior.
On 2/6/07 around 11:30am, the #1 dryer was shut down to make some adjustments on the PLC Controllers. Around 11:45, plant people noticed flames coming out of the ID Fan Piping, at that time we got the fire hose located in the dryer room and began to spray the fire. The fire dept was called out to assist, fire was extinguished around 12:20pm. No one was hurt.

ROOF FALL (7000-1-01-07) : LOCATION-1W#5, X-cut #98 between #4 & #5 entry. DIMENSIONS-H:6-7', W:19', L:40'. GEOLOGY-soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT-5' fully grouted bolts.
Location: Section #1, entry 3, spad no. one break outby spad 222; Distance from face:2,100'; Airway: neutral; Size: L-15'; W-15'; H-3'; Type and length of roof support: 48"" resin fully grouted; No cribs and timbers installed prior to fall. Strata: shale, sandstone, mixed, laminated. No coal rider seam present, all entrances to fall are being breakered off.
An unplanned roof fall occurred atr the 54 mains section in the #6 entry at spad #2365. There is no active facem these are old works. No injuries were reported. No equipment involved. Ventilation and passage of persons were not affected. Fall measured 40' length x 18' width x 4' height.
A fall of roof has occurred in the 2A Panel between x-cut 38 and 39 in the # 1 Entry. The Fall was approx 30' long x 18' wide x 8' high.
An unplanned roof fall occured at the 54 main section in the #2 entry 3 x-cuts outby spad #7449.There is no active face, these are old works. No injuries were reported, no equipment involved. Ventilation & pass of persons was not affected. Fall measured, 40'l x 18'w x 4' h.
A new roof fall was found int he #1 entry of South Mains ""B"" return aircourse between 23 and 24 x-cut. Fall is approx. 18'wide by 8-9""high by 20-30'long. Area bolted with 8' Install III bolts with resin assisted and some floor to roof support. Cause of this fall was deterioration of the main roof due to weathering. Fall did not affect ventilation or escapeways.
Fall occured on No.4 belt linea t brk. 16 approx. 6' - 8' high 20 X 20 in intersection.
Power outage at the brewer shaft site. The employee diesel air compressor would not start due to the freezing weather conditions, leaving personnel trapped underground for more than 30 minutes. Power was restored by 10:30 a.m.
No one was injured or any equipment involved. We have soft top conditions and low cover in this area. No cracks or water was present in this area during normal mining procedures.
A roof fall was discovered by the fire boss while making air ways. The fall was approximately 20' wide by 6' high by undetermined length. The fall will be breakered off and will not be cleaned up.
Mains 1 right section, MMU 007, 4-way. Support: 6 ft. 3/4 inch resin bolts, 12 ft cable bolts, pans, brow straps. 20 ft long x 18 ft wide x 8 ft high. Fall dangered off, additional roof support installed around fall.
The Miracle Run automatic doors would not open. It was determined by the Shindler Elevator repairman that the door opening motor was bad. The motor was changed out with the elevator returning to operation @ 12:30pm on 2/5/07.
Main Portal Elevator was out of service. Adjusted lower door landing switch and elevator was put back in service at 7:48 p.m.
Operator of HT 416 was told on the radio by another haul truck driver to check his wheel motors, as the covers were glowing. The operator got off the truck to check his wheel motors, but failed to shut the truck down or activate the fire suppression system. By the time he got down off the truck, the fire was out of his control.
The Cedar Creek Elevator was out of service from 10:35pm until 11:30pm due to door switch problems.
The #3 coal mill was slowly being brought back up to full production rate when an explosion occurred in the coal mill circuit causing the pressure relief explosion vent to rupture. The shift supervisor had opened and then closed the tramp iron tipping valve to make sure it was operating correctly and appproximately 10 seconds later the explosion occurred.
A roof fall occurred on the 5 West Mains. The fall was located between entry 4 and 5. This area was between the roadway and belt. The area had been noticed by examiners about 2 months ago and cribs had been erected on both ends of the crosscut prior to the fall.
A fall of the roof occurred on #6 unit in the 3rd N.W. Submains at x-cut 9 in the #6 entry. The fall measured 80 ft x 20 ft x 10 ft high. Personnel, ventilation or equipment were not affected.
The #3 coal mill system went down loaded with coal due to a secondary firing fan motor failure at 7:45AM. The motor was replaced, and when the operator began to ""bump"" the coal mill empty several hours later an explosion occurred in the coal mill circuit causing the pressure relief explosion vent to rupture. No employee injuries or equipment damages occured.
A piece of rock approximately 40 inches wide, 8 feet in length, and 10 to 16 feet high fell from a faulted area between two rows of primary roof bolts. The fall occurred in 21 Headgate, entry 1, crosscut 32 to 33.
A fall of roof occurred in the 2A Panel between x-cut 45 and 46 in the # 6 entry. The fall was approx 50' long x 18' wide x 8' high.
A fall of roof occurred in the 2A Panel between x-cut 44 and 45 in the # 5 Entry. The fall was approx 30' long x 18' wide x 6' high.
unintentional roof fall 3rd south sub main #2 entry 3760' mark. 4way intersection mined approx 1/26/07. Bolted with 6' & 8' tension rebar bolts 30'l 18'w, 9'h. Called in 2:25pm 2/2/07
A roof fall was discovered on the B-Mains #2 Belt at the 39th break. The fall was located in the intersection and measured approx 20' X 20' X 10' in thickness. The area was originally supported with 5' High Tech Bolts and 8' Super Bolts.
The continuous miner was pushing cleanup matrial after completing a cut when a flame appeared at the miner head. There was no damage to personnel or equipment.
A roof fall was found in a worked area by our weekly examiner. The fall was located on the 1 left, 4 West, 2 right, 3 left panel. The fall was located in entry 6 at crosscut 18. The cause of the fall was weak laminated shale.
An R-35 Euclid Haul truck and a rail car collided.
The truck owner was replacing a bolt in a pony axle. The owner had a bottle jack under one axle tire and a wooden block under the axle tire beside it. The owner then lowered the axles from the cab, positioned himself on a creeper between the 2 raised axle tires. The jack came out from under one axle tire; crushing the owner.
EE was assisting in changing 4 sections of 30 foot steel on an Ingerso Rand surface drill. One section was stuck and when it freed, it swung approx 4', striking the EE's right hand. It caught the middle and index fingers between the steel and the drill head. Amputation of the two fingers between 1st and 2nd joints.
Employee was adjusting forks on forklift when his right ring finger got caught between the fork and mast. The end of the finger was severed off.
Employee was walking to job site when he stepped into 8 to 10 inches of mud. Employee was unable to avoid the mud.
While loading tools on Mac 8, the Mac 8 started to roll and ee hit left elbow on frame of Mac 8.
Employee was helping shift mechanic put bumper on 11 CM; mechanic hit keeper pin with a 4 lb. hammer. This caused a piece of metal to come off pin head and fly into employee's neck.
Employee was splitting stone when a large slab slid from the top of the pile and pinched him between two rocks.
Employee was loading cable on pressure pump when he slipped and fell.
Backed over a big rock, hit hard and jarred lower back - did not seek medical treatment until 3/2/07.
EE told supervisor he felt small lump in chest & went to see personal dr. Xray revealed 1 suture from prior surgery undone. EE released w/weight restriction. Company did not feel this was work related. 3/15 EE went to see different dr., because his was out of town. Dr would not allow EE to return to work w/o stress test. EE has not done this.
Member was welding on a bit lug on a continuous mining machine. Resulting in a burn to both his eye. was not reported until 3/15/07
EE was installing roof bolt in the top, when a piece of draw rock fell from between the TRS& Canopy, hitting him on the back and right knee.
Using grease gun & reached between belt & roller to wipe fitting. Caught glove & pulled in hand bruising finger & hand.
Employee was walking to his car in the parking lot and slipped on some ice and fell jamming his shoulder. The employee continued to work. After a followup visit with a physican it was determined that surgery was required. On 4-2-2007 surgery was performed. This was the first day away from work.
While walking, twisted right knee. Surgery required.
EE bent over to roll stone out of the way, when he felt pain in his lower back.
Crew was hoisting a box of parts in the machinery well in the plant. Injured employee was on floor helping guide the box thru the floor opening. Injured employee signaled to another employee on the floor above the raise the box. Injured employee his hand on the sheave cable, left hand got caught mashing the ring finger and small finger on the left hand.
Employee was operating D10N Dozer, when he backed up over a rock, injuring his back.
Employee was replacing a guard on a conveyor. He was sent after a nut and washer. The employee turned, walked into a hanging ladder from the conveyor and fell against a concrete support. This fall caused injury to the employee's ribs.
EE was walking down entry, tripped on a gunite ledge and fell. He put out his arms to catch himself, struck flat car with left arm and shoulder.
Employee stated he was using the boom of the bolter to push bolt into a hole and his thumb got caught between the bolt head and the boom causing a crushing injury to his left thumb.
Employee was training RR Belt at belt tail, when a piece of coal fell from catwalk above, striking his arm just above the elbow.
Roof bolting, a piece of draw rock broke and hit left ring finger.
EE was welding a brace on logwasher. EE placed 6' (2x8) board on 3rd rung of ladder and guardrail of logwasher, (unsecured)for platform. Scaffold board slid, causing him to lose his balance and fall into the logwasher, approx 44"" from bottom of the board.
Driver stated he got into berm and went into ditch while driving the coal truck.
Employee was preparing to get up on a loading machine. As he started to put his foot on the first step, he somehow missed the step which must have thrown him off balance. When his foot came down on the ground, the strain apparently caused a hernia in his groin area.
Employee was using a hammer drill to place anchor holes in concrete. The drill bit became bound causing the hammer drill to twist in his right hand. He visited Hickory Orthopedic Center on 03-01-2007. Review of his roentgenogram confirmed a malrotated, oblique, distal diaphyseal of his third metacarpal.
He was backing up a D-9 Dozer, ran over a rock. He said it pulled his neck.
992G #L27 was loading in shot at Hollylick Pit #64. He was digging rock. While prying rock, loader bounced up and down. Employee said then that he hurt his back. This took place about 7:50 PM on 2/27/07.
While in the process of dismounting the prill truck to clean out the auger, employee did not realize that he was parked over a depression. He tried to catch himself and strained his right shoulder.
Employee stated that he had filled the dozer with fuel and was standing on the track of the dozer when his foot slipped and he fell off of the dozer landing on his back on the ground. He went to the emergency room and x-rays did not show any broken bones and the diagnosis was bruising only.
Injured was fixing lights on a Benson trailer #132 standing on a ladder. He slipped and fell off ladder onto a concrete pad. Took to hospital by supervisor. Has a slight concussion to head and hurt right elbow.
Employee s foot got caught between ram car center section when abruptly moved.
EE was attempting to change the transducer on shield #117. After he lowered the shield, he shut off the pressure and released the pressure on the transducer. As he pulled the stape out of the coupler, the hose still had pressure on it and when it was released it injected emulsion into his #4 finger on his left hand.
Employee fell on ice exiting his vehicle in the parking lot. He fell on his back. He refused to be medically transported. Person worked the 27th of Feb, and reported off the 28th. Returned 3/6/07 - total 4 days last time.
Employee was lifting an ATRS pad and felt pain in groin area. He was diagnosed with a hernia and started missing time on 3/6/07.
Employee was tearing open plastic bags that had holes in them ,when he said his wrist & hands (Left & Right) started to lock up .
Stepped out on dozer Track's and slipped and fell off deck of dozer bad weather of ice.(Catwalk)
The employee was walking in the beltline and stepped in a hole. The heel portion of his boot went into the hole and his foot twisted.
Employee was standing next to the scoop bucket when the scoop operator lowered the scoop bucket on his foot.
Employee was moving an empty oxygen cylinder. He pulled and tore his bicep muscle. The muscle was surgically repaired and the employee was placed off work.
EE was attempting to change a blow plug on the fluid coupling at the G.H. 2 Belt Head. As he was turning an Allen wrench to loosen the plug, the wrench slipped causing him to strike his right wrist against the metal framing. EE ceased work immediately. X-rays were taken and were negative. Due to the excessive swelling he was removed from work.
Employee (roof bolter operator) was preparing to drill a hole in the roof for a roof bolt when his foot slipped under the drill boom while he was lowering the boom, catching his foot. The area was dry with a small amount of loose coal.
Employee was in the process of moving a rock out of the roadway of the powder truck when something snapped in his back causing severe lower back pain. He started missing work on 3/2/07.
Rock discharge from crusher, hit conveyor belt and bounced striking employee on side of hard hat.
Reported 3/8/07-foot slipped on the bulk truck, fell and broke 3 ribs.
Slipped on coal when he was carrying buckets of sealant and hurt left knee.
Feet got stuck in mud while carrying posts. He hurt back.
He was exiting the #7 compressor room when he slipped on snow covered ice causing him to twist his left knee. He continued to work at his regular occupation and eventually went to a physcian who diagnosed him with a possible medial meniscus tear. He is scheduled for surgery on April 4, 2007.
EE was installing roof support, the ladder he was standing on slipped and he fell in the opposite direction. EE landed on a piece of metal which cut into his left ear resulting in 12 stitches.
10 H headroller, walked from beltline, into the track entry, he tripped over a corp board, causing his left foot to twist outward. This twist put strain on his upper leg (fibula) causing a fracture. *lost work days began 2/26/07.
EE was coming down stairs @ end of shift, bottom of stairway under construction. 9"" steps at bottom of stairway, had to step up onto a 13"" I-Beam, stepped off I-Beam and on the ground were two pieces of steel conduit. EE stepped on conduit, the conduit rolled and EE twisted left ankle and fell to the ground.
Employee was bent over explaining something to another employee. He stood up and struck his head against a well marked, low hanging, gas regulator. Employee complained about a sore neck the following day.
Employee was beating a bit insert back in the miner when the handle came out of the hammer causing him to hit his left hand on the bit lug.
Employee was draging a section of dust line. As he pulled on dust line his shoulder poped out of place.
Cleaning tailgate area. Right index finger smashed.
Employee was exiting a building when he slipped and fell on ice/snow, fracturing his left wrist.
EE has made a claim under our worker's comp alleging that he injured his back while lifting a 50 lb bag of block bond on 2/24/07. He had not made any mention of injury that day to his supervisor or to his co-workers. It was 12 to 14 days before we learned he was seeking treatment. I then had to wait to hear from him to get details as he had had his phone number changed.
While cleaning area employee was lifting a Barrel to dump waste and felt a small pain in his Back. Employee continued to work until 3-1-07 and Dr took him off work for at least 1 week.
Employee states that while cleaning an auto splitter between loads the lid of the splitter fell onto her left hand/wrist causing a contusion.
Employee was loading roof bolts he picked up a bundle of plates. He slipped and fell on plates cut right index finger.
When the employee removed wheel block from behind wheel of his truck it began rolling backward. While attempting to get into his truck he failed and became pinned between his truck & another which was parked adjacent and behind.
EE was running Micro Traxx, cleaning out ash in combustion chamber of dryer, when machine broke the ash loose causing machine to kick sideways. This caused EE to hit middle of his back on the wall of the dryer chamber.
Employee was bolting and a rib slab fell and hit him on the right shoulder.
EMPLOYEE STATES HE WAS STEPPING THROUGH BELT WHILE SERVICING THE DRIVE WHEN HE MISSED STEP AND FELL TO GROUND RESULTING IN FRACTURES.
Walking on snow covered ground, slipped, twisted knee & lower back. Did not seek medical attention until 2/26/07 - Disability started 2/27/07
Employee was moving a used pallet. He was lifting the pallet and reported having a sore back. He has a strained low back. This is a reoccurrence of an old low back injury.
EE was crawling beside a cutter machine checking for a leak and the tire pinched his right leg.
The employee was stepping down from a Loci and stepped on a loose rock along the rib turning his ankle.
Employee was putting a 5' resin bolt into rib when rib rolled out striking empoyee from behind in neck & shoulders.
Setting up saw tracks, stepped in hole.
Employee was rock dusting beltline. He was walking backward and tripped over a roller in the walkway.
Employee was walking behind a maintenance sled when he slipped and twisted his ankle.
Injured party was pushing rock with a bull dozer, when the dozer slipped off the road, jarring the injured party and hurting his lower back.
Did not start losing time until 2/28/07. Stepped up on box and felt pain in back of his left knee. Surgery required.
Tightening wheel on truck with lug wrench, strained abdominal muscle.
While sliding a heavy steel member on table, EE pulled something in lower back. Wrote it on 1st aid board & con't to work. On the 27th & 28th, he tweaked it again & told supervisor. Dr. told him light duty, which makes this a lost time because of shop policy. He is supposed to get an MRI.
Employee was working under the pan feeder of Crusher 2 clearing rock off a concrete ledge so a skid steer loader could scrape the material out of the pocket. The skid steer scraper attachment made contact with her left ankle causing a laceration.
Traveling up belt line, when he stumbled and fell, twisting his left knee.
Miner was putting up utilities off of scissor deck. He did not put chain across the back of the scissor deck, slipped and fell off scissor deck landing on his butt. He has a compression fracture of his tail bone.
Employee picked up miner cable to move it out of roadway and then complained of back pain. Employee slipped in mud while moving the cable.
EE stated he was installing outside bolt using 2 piece's of 4' SRD bolt. He started to shove the bolt up in the hole and pinched his thumb in between the SRD coupling and the mine roof.
Employee was cleaning the belt take up roller on the plant feed belt with a hammer. His right hand and lower arm got caught and pulled into the roller. This event is under further investigation.
Employee was entering plant and stepped on loose coal causing right knee to twist.
While maintenance was being performed on equipment with cutting torches and welder. Some hot cutting went down inside top of boot and burned top of left foot.
Unit #3 - Employee was changing leaking hose in #137 Hauler. Slipped in oily bed. Caught self with right hand on side of bed to keep from falling. Incident resulted in a right wrist sprain. EMPLOYEE DID NOT LOSE TIME UNTIL 02/26/2007.
Employee was lifting 50 # bags from packaging machine from a North position turning and stacking at waist level to a South position or 180 degree turn.
Employee was standing on top of Pepco Chute Car helping to move it. When car was moved, employee pinched his left ankle between a steel I-Beam and a steel channel.
On 02.22 on employee's way home from work, he rubbed his L eye, & felt like something was in it. At home he checked it & did not see anything in it. On 02.23 when he woke up, his L eye felt scratchy when he blinked. He washed his face, then felt okay & he went to work. On 02.24 when he woke, his L eye felt scratchy, it was red & irritated, & he also saw something in his eye.
Employee was carrying canopy post for roof bolter when he set it down and felt something pop in his lower back. Started physical therapy on 3/2/07.
Freeing gun steel with comalong - clevice broke causing chain to loosen.
Left hand was pinched on scoop.
EE was working on a loader that had its cab & transmission removed. He was where the transmission had been, cleaning the area. He raised an access panel on the hood to give himself more room. His hard hat fell off. He stood to replace it, the wind blew the access panel shut striking him in the head before he could replace his hard hat. He required 4 stitches, 1 day off work.
Employee was welding and grinding throughout the day. A particle lodged in his left eye and was irritated.
He was moving 2 4' and 2 8' x 16'' Symons Forms out of the way for the ironworkers and slipped while lifting one of the forms due to cement dust on the mud slab. Mud slab was level, but dust-covered and he slipped in a puddle or on a slick spot. **Employee did not notify employer until 3/6/07.**
EMPLOYEE STATES WHILE WALKING ON ICY ROAD, HE FELL TWISTING HIS LEFT ANKLE, RESULTING IN A FRACTURE.
EE and co-worker were removing stopping when a rock measuring 22"" in length by 15"" in width and 5"" thick fell and struck him on the head causing a fracture in his T-3 vertebrae.
Injured was opening air lock doors when he strained back. did not start missing until 2/26/07
Employee was pulling cable when he felt burning and discomfort in the groin area. Lost time injury as of February 26, 2007
Employee was operating a packing station when it became clogged. As he was reaching to to shut off the machine, dirt hit his left hand and employee sustained 2nd degree burns.
Twisted ankle on rock coal and sprained ankle.
EE was cleaning build-up of frozen material from underside of conveyor belt, when it let loose and fell against his leg.
Dozer was pushing mateiral down a slope for reclamation. When he ran over a rock causing dozer to loose traction and slide sideways. Dozer then turned over.
Crusher operator stepped on rock causing left ankle sprain. Reported to supervisor on 2/22/07. Doctor issued work restrictions resulting in lost time. Employee expected to return to work full-duty on 3/5/07.
Steam Cleaning heavy equipment, crawling around on ground, knelt on rock.
At RNS Barnes/Watkins site on 2/21/07 employee was operating a rock truck when he hit a hole in the haul road. His upper body was twisted in an odd way causing pain to his upper back. He did not know the severity of the injury at the time and did not seek medical attention until 3/01/07.
While operating D-9L dozer on shot rock - operator reported back pain.
Belt was being installed. A loop of belt caught the shuttle car cable that was hung up and it hit and knocked employee down. He has been diagnosed with a fractured tailbone. Started missing work 2/22/2007.
Working on the T-block taking a piece of marble off top of rock when he lost footing holding marble and fell to the ground.
Scoop was pulling belt around the rib and the belt caught a shuttle car cable and pulled it down. It struck employee and knocked him down. He has a bruise on his back and soreness to his back. He started missing work 2/26/07.
While walking through the plant yard the employee slipped in mud causing him to fall. Originally diagnosed with hip strain, an MRI performed in mid-April showed some bone fragmentation, which was removed laparoscopically on 5/15/2007.
While employee was performing his normal duties as a Roof Bolter/MBC Operator in low coal he experienced pain/swelling in his left knee.
Employee was standing on the ground while tightening a bolt on the loader bucket. The wrench slipped off the bolt sending employee to the ground. Employee broke his fall with his left wrist.
Employee stated that he was loading scoop when he slipped in bucket injuring his right ankle. Employee went to Dr. two days later and was taken off work beginning 2/27/07.
Employee was using a stone cutting saw when he was not following proper procedure and placed his hand in the path of the blade causing a laceration to his right hand.
Employee got his finger caught by the auger of the rock duster which resulted in a laceration to the right hand tip of middle finger.
Employee was in the process of soldering a ground wire in a heater. He did not notice that his right pants leg was too close to a fuel oil space heater and his right pants leg caught on fire. This resulted in second degree burns on his right calf and slight burns to both hands.
He was cleaning up some mud with a shovel. He said his knee popped and he felt a lot of pain.
Employee was working at Unit 2 Kiln cooler dump point. While they were observing pellets in the cooler from the area of the access doors, a blast of hot dust and steam from inside the cooler knocked open the steel doors and pushed the employee backwards. The hot dust contacted him causing first and second degree burns to his head and neck area resulting in loss time.
Employee brought scoop to outside for supplies. When he started to get off scoop he caught his left ring finger, causing a laceration to tip of finger and nail. He went to Pike E.R. to get stitches.
Injured sprained his left ankle when he stepped on uneven ground after dismounting his haul truck.
Employee was coming out of the front gate when he turned his ankle. He was taken in to the er. Employee was taken off work. On going.
EE had upper body thru access door to primary screen sand bin to assist co-work in dislodging massive chunk of sand, ice & snow was frozen to sides of bin overtop of moving conveyor belt. During their attempts, belt caught chunk & suddenly moved it towards ee. Frozen material struck ee and wedged him against access door frame.
Injured was delivering belt structure. As he was removing a piece of belt structure from the scoop blade he felt pain in his lower back.
A coal rib fell and struck Employee on the left side injuring him on the left side shoulder neck area. Resulted in stoving him up.(Bruising)
Ice & snow covered highwall, thawing. EE was unpinning auger flite approx 4' from highwall. Big rock fell from highwall, hit auger, rock guard, pipe, split, 1/2 rock hit EE's left leg @ knee area.
He was helping a mechanic lift a contactor case in place on a loading machine and felt pain in his right shoulder. He was diagnosed with a right AC separation.
Employee was in the process installing a 6' fully grouted roof bolt. At this time he swung the boom of the bolter out to put up his rib bolt and inadvertently hit the wrong lever and dropped the boom down onto his right foot causing a deep bruise to his foot.
Installing a regular roof beam in a wall. Took block out of wall, attempted to make a better fit. Sat block down on bottom portion of wall, when beam fell striking left thumb between block and beam.
Shoveling mud out of take up at #4 head-drive, injured left shoulder. Return to work 2/21/2007. Worked until 8-14-07, had surgery on his left shoulder 8-15-2007.
Employee states while removing a plug from the high pressure port on a shield, the plug blew out striking employee on the chin. Employee received a fractured jaw, laceration and chipped teeth.
Breaking loose fozen coal from belt structure and hopper with a pinch bar. Material suddenly dislodged, pinning left hand between bar and structure. Hematoma of the left hand.
Injured was stepping across a steel beam lying on the bottom. When he stepped down he stepped on a rock and turned his ankle.
Employee states he was pulling filter cloths and felt pain in his lower back/leg.
Employee stated that he lost his balance and fell down stacker belt steps, ER visit, Dr. removed him from work.
He hit a low area in the road which caused him to bounce around in the haul truck and hit his head and elbow on driver's side window.
Taking a nut off a pump with a ratchet, pulled on ratchet and felt pain in left shoulder.
Employee did not report any incident until 2/20/07 from doctor's office. He stated that he had experienced shoulder pain for ~ 1 month prior and it was work related. This employee lifts and stacks 50# bags of clay.

EE was using a long lever bar to gain mechanical advatage to turn over a belt scrapper. The bar slipped, striking the EE in the jaw.

Employee reported: Working as a brattice man building a return stopping on the no. 3 unit. When completing the task, lifted an 8"" hollow block, turned to place in the top row of the stopping and experienced pain to his low back.

Employee was loading his 2007 Mack fuel truck with diesel fuel at the diesel fuel station. He stepped in the cab on the passenger side to warm his hands. As he got out of the vehicle, he slipped on a step, suffering a compound fracture to his right leg.
EE was positioned between #3 mobile bridge and the low-low structure, right foot on the low-low belt, left foot on mine floor. EE was attempting to bump conveyor chain, in this process, his arm or shoulder engaged the right tram causing the bridge to move forward, catching his left leg between the mobile bridge and low-low structure. Fractured his fibula and cut his calf (sutures).
Employee was walking from left miner to right miner & tripped & fell over some loose coal. He hurt his right elbow & twisted right ankle, pinky finger & left wrist. He returned to work without seeking medical attention. He called on 26th of March & said he couldn't come to work because the doctor said he had a sprained ankle.
Employee was cleaning around an underground conveyor belt drive when he slipped and fell, twisting his right knee. He did not receive medical attention until 3/4/07 when he visited the local E.R. where x-rays were performed but did not show any damage. He was advised to follow up with his family physician and was taken off work for further testing.

A Granite construction employee was working on our job site. He was working with our employee investigating & fixing an electrical issue inside a portable electrical panel box. Employee was assisting in replacing a hydraulic jack on the continuous miner. The jack slipped, catching the employee's left little finger between the jack and the miner frame. This resulted in laceration to the left little finger.
Putting scoop on charge. As he pushed charger into batteries, his feet slipped and he started to fall. He tryed to catch himself, grabbed scoop bucket and cut his left hand between thumb and first finger. 10 Stitches.

Member was in the process of mounting his coal truck. When he grabbed the handrail to pull himself up, he stated he felt a ""pop"" in his right wrist.

EE rec'd cortizone shot on 5/31/07 making this reportable. After continuous treatment he had surgery on Jan 7, 2008 making this a lost time accident.
Personal Injury - Hyperextension of Left Elbow. Employee was moving rock on rollers at Chopper; rock suddenly stopped causing left elbow to hyperextend. Came to office where injury was iced; taken to hospital for determination of extent of injury. Doctor placed him on 7 days of light duty.
The employee missed a step as he was descending a set of steps. He fell 2-3 steps and struck his knee on the landing. The landing consists of metal grating.
Employee stated that he was attempting to stand from a prone position, as he stood and moved to his right side his knee twisted and popped. He stated that he immediately experienced pain and swelling.
EE tried to avoid bag hitting the ground when it came off the belt line. He caught the bag with his left hand, turned left to put the bag on the pallet, pulled back muscle.
Coal hauler was rolling up tarp on trailer manually and the crank came back and struck him in the chest causing contusions to chest and right hand. 1 day missed work.
EE was hand loading a section of ICA track onto a flat bed trailer when he felt a sharp pain in his groin.
Employee alleges he was walking down the belt line; he stepped on a roller and fell. He was having pain from his hip to his foot. This became a reportable accident on 3/22/07 at which time the doctor determined it was work related.
Member had been cutting strings off of bottom hanger brackets when a belt splice caught his glove pulling his left arm into the belt. He has continued to work but did get a skin graft on 3-9-07 and is now unable to work because of the risk of infection to these areas.
Injured employee had stopped by the Warehouse prior to the start of his work shift to pick up some ear plugs. As he was walking between his personal vehicle and the Warehouse, he stepped on some ice and fell on the parking lot surface. The fall resulted in a fracture of his left femur bone.

Injured EE and another EE were drilling an anchor hole in the mine floor to anchor belt tail piece with a hyd drill. The augar got entangled in a tool pouch, hanging from the other EE's belt, causing injured EE's fingers to become twisted around the handle of the drill. This resulted in a fracture of the middle and ring fingers of the right hand.
Employee was in the process of lifting the back plate pulley assembly to the take-up unit at 4 head with a 3/4 ton Harrington chain hoist. The hook rigging failed, allowing the assembly to fall on his right foot, causing a fracture of his 7th metatarsal bone.
Employee was in the process of lifting wire mesh panels into a pickup bed for transport the wire mesh panels are 4ft by 8ft and weigh approx. 10 lbs each.
While walking to plug in a block heater on the tractor he fell on some ice. He did not complain until recently when he was sent to a doctor and a problem was found. He began missing on 04/29/2008 for surgery.
3 mech's were disassembling a Crane. The pennant lines were still connected to the boom and the tip was blocked. The injured removed the lower pins from a section of boom not realizing that it would cause the boom to move in a ""hinge type"" motion. The boom hinged catching his foot between the boom section and the ground. Resulting in 4 broken bones to his foot.
The employee slipped on ice, aggravating a non- occupational left shoulder injury. Her next scheduled work day after the accident was 2/18/07.

EE was scraping ice from windshield, EE slipped on catwalk and fell to the ground from the cat walk, on the 777 Cat Dump Truck.
EE was replacing a starter in the MCC room. EE was removing the ground wire from the damaged starter when the wire accidentally touched the back of the starter next to it; there was a flash/spark. EE complained of seeing spots but continued to work and finished his shift. The next day EE informed us that his eyes were bothering him and he was going to see a doctor.
EE & co-workers were moving section power. His assignment was to move up the FCT power cable. This task required pulling the slack along the belt entry then extending this cable across the low structure belt. When pulling the cable along the entry, he stumbled over a rock, twisted body & experienced pain to low back region. Continued to work until 2/27/07 & began missing work.
While walking into sump area to check pump, slipped and fell on mine floor injuring left shoulder. Did not seek medical care. Individual then saw doctor for pain on 4/23/07 at which time Doctor prescribed therapy and to stay off work for a possible torn rotator cuff which may need surgery.
While attempting to place crusher main shaft, employee's gloved hand was snagged by the slag left from cutting off the pad eye, pulling him slowly in the direction the crusher main shaft was turning causing him to grab with his other hand to keep from losing his balance. This caused the jerk on his left arm and the resulting injury.

Installing arches on 2 1/2 foot centers under supported top in 10 west longwall headgate areas for longwall recovery. Employee was installing bolts in arch when rib fell hitting him on side of head. 4 to 5 inch laceration on side of head.
Employee was operating roof bolter when a piece of rock fell from the roof, strking his right hand.
The employee felt pain in low back and right hip while engaging a line switch on the trolley line.
Miner was in 6 Rt, while cutting middle band of rock, the continuous miner started to bounce. The miner boom was bouncing up and down, causing the load-lock retrieve valves to release and let the boom fall into the S/C. This caused S/C to bounce and throw EE, the S/C operator, up into th top of the canopy.
Associate was stepping down from drill carrier cab onto the ground. He landed hard on his right heel. Experienced some discomfort at that time, but it did not bother him the rest of the day. The next day his heel was swollen and painful and he went to the doctor.
Employee was working on lifter holes at the face when a rock fell and struck his back. He received two fractured ribs from the impact.
While moving a fire extinguisher, one accidentally went off. Employee breathed some of the powder.
EMPLOYEE STATED THAT HE WAS WORKING BUILDING A SEAL WHEN HE TURNED WITH A SOLID CONCRETE BLOCK IN HIS HANDS AND STRUCK ANOTHER BLOCK WITH HIS RIGHT HAND. HE WAS X-RAYED AND IT WAS DETERMINED THAT THE TIPS OF HIS RIGHT MIDDLE FINGER AND HIS RING FINGER WERE BROKEN.
Employee holding timber while celing capped, a piece of rock fell and struck employee on the back of neck.

Inj EE lost his footing on catwalk, holding on to hand rail at door post, body weight jerked hand loose, pulling & straining rt shoulder & fell onto track, push arm and onto ground. Not sure which hit caused lower back injury.

Unit #1 - Employee was rib bolting off of the scoop with a rib drill gun. The auger hit a rib bolt on the other side, causing the gun to twist resulting in a strain to the left hand, wrist, and forearm area.

The fall resulted in a fracture in the upper femur. 6"" of snow fell during the night and a Bobcat was removing snow in the parking lot. Employee was walking down the path that the Bobcat had removed when he slipped. Employee intended to retire on 2/15/07.
While stepping down off water truck placed foot in a low area on ground, spraining ankle and causing back pain.
He said he was walking the belt line @ #1 belt & 1 break inby #A head. He said he was looking at the belt roller & allegedly stepped on an anchor pin & turned his foot. He left at end of shift & didn't report alleged incident/injury to immediate supervisor. He said he went to hospital 2/13 & had his right foot xrayed & no breaks were found. He said he was referred to another dr.
3W#3 Belt - Employee overextended right shoulder while pulling belt block across belt preparing to make a splice. The right shoulder did not pop but, employee complained of severe pain. Strain to right shoulder.

Removing pump impeller from barge pump with a Pettibone crane. Impeller came loose and rotated causing the chain hook to come off the vane striking him in the sternum and throwing him 6 ft and landing on top of fellow employee knocking them both to the ground. As a result, the employee received a fractured sternum.
Employee was standing in scoop bucket and slipped on wet foam twisting his left knee.
Employee's scraper came to a sudden stop when the slack was released after the hook up. Afterwards, employee began to feel some discomfort in his neck and head. He was transported to the ER where he was checked out. He was taken off work. RTW 2/21/07.

Employee was bolting roof after a shot was fired. He was moving a jackleg when he slipped on the muck-pile falling on his left knee. This resulted in a contusion to his left knee.

Employee states he was working with his supervisor getting a motor ready to be pulled when he felt pain in his low back and down his right leg. A Notice of Contest was filed on this claim pending investigation. Based on litigation, this claim has been accepted as compensable.
Employee was operating the left side of a dual boom bolter when a piece of draw slate measuring 3 feet by 5 feet by 8 inches fell from between the roof bolts striking him on his shoulder and leg.
Was cutting trees & tree split, man slipped on icy ground and fell. Then, tree jumpede off stump and onto man's left leg. This was a contract tree cutter.

Digging rock material from conveyor belt tail pulley, tool was a shovel. Injured on 2/9/07 and came right back to work, went back to the doctor on 3/5/07. Doctor scheduled surgery for 3/8/07. RTW on 3/28/07.

An unknown mechanical failure occurred in the Clyde Frame 10 hoist being used for shaft steel construction that allowed the work platform in the shaft to briefly freefall prior to being stopped by the overspeed protection on hoist.

Employee was firebossing for a construction company. He and three other workers were coming out of the shaft in a construction bucket when the bucket suddenly dropped aprox. 10 to 30 feet and then immediately stopped causing his legs to be driven against the bottom of the bucket and broke both his legs below the knee.
Employee was walking down the stairs at the mill maintenance building when she slipped and fell landing on her back. This resulted in a strain to her neck and back.
While jacking a wrecked car back onto the track, employee caught hand between coupling and supply car.
Employee was assisting with welding of lagging onto head pulley for a conveyor belt. He was attempting to hold lagging when the welding rod contacted the head pully causing an arc. He was not wearing eye protection at that time because he was not expecting an arc. Employee felt the effects of flash burn to his eyes after he got home from work later that day.
Was setting steel on pot when rock fell hitting thumb. Rock measured 4"" thick x 18"" round.

Employee was bolting top. Rock fell in front of ATRS. EE started to run striking head on personal canopy. He worked up until 3/23/07 & then started missing work.
Employee stated that while descending the steps of the Rotary Dump he tripped and fell, feeling pain in his left shoulder.
Employee was walking to scale house after completing his shift as dredge operator. He misjudged the depth of a pothole and claimed to wrench his back. He called at 11:00 a.m. to report the accident and said at that time that he would be going to a doctor and filing a Workman's Comp claim.
Ee was pulling on conveyor belt and felt a pain in his groin area. Diagnosed with a hernia. Started missing work 3/16/07
EE was using a shovel to remove partially frozen Ag lime that was stuck in the bucket of the loader. He believes he strained his back at some time in this process. He did not notice any pain at this time. He had pain when he got up in the morning of 2/8/07. He came to work and advised that he couldn't climb in out of the loader and was going to see his personal physician.
The employee stated he had scaled a rib down and while walking around the bolter he slipped and fell.

Emp stated he was lifting a piece of lumber from the 5th floor of the RSP Tower to the 5 1/2 floor. When he slid his hand down the board to get a better grip, a splinter from the wood punctured the left palm. Emp was wearing leather gloves at the time of injury. His first lost time day is 2/8/07.
EE had just finished hanging tubes and began bolting when top started to drip. He turned and headed toward tail of miner to get away, when a piece of rock came down and struck him on top of left hand. He suffered a puncture wound on top of the left hand, which required 4 sutures to close.
Employee was walking down steps on an 785 rock truck. He slipped coming down the steps hurting his left shoulder. Employee continued to work until April 26th when he was taken off work.

He was stripping concrete forms and while lifting form felt a pain in the groin area.
While fueling bulldozer, operator slipped off track to ground approx 8', landing on left ankle. Operator was pulling on fuel hose prior to slipping.
Employee was lifting a heavy stone. The stone was icy and his gloves were slippery because they were wet. The stone slipped out of his hands and when he tried to catch it, he felt a tug in his back. Low back strain. Restricted duty.
EE was setting nail gun down (while building pallets) and bumped his left hand and drove a nail into it by his thumb.
Employee was driving out a bolt using a welders chipping hammer instead of using a punch. A sliver of metal became imbedded in his left hand.
Removing a drain line. Curring line on 4th floor and victim was steading pipe on 3rd floor. Victim was warning other worker and looked away and pipe smashed fingers.
A piece of rock popped away from the roof/rib area striking the employee in the leg and knee area causing some swelling.

On February 6, 2007, employee was getting out of truck, when he slipped and fell twisting right leg.
Employee was traveling on mantrip looking for scoop. He backed up to let another vehicle pass. As he moved forward he struck a rib causing the steering column to move out of control. The steering column hit his right hand causing fractures.
He was changing tires on a 10 wheeler and twisted his back.
Employee was burning empty parts boxes after repair when an empty ether can in one of the boxes blew up burning employee's face. (Local MSHA office at Hazard KY was notified immediately.)
Employee was stung by a bee as she walked to the front gate. The bite site of right arm got infected and was treated. Ongoing.
Slipped and strained his left knee in the 5A 5 North. First day missed was 2/12/07.
Man was putting up 10' pins, rotated hand and wrist around pin and hurt ring finger and little finger of left hand. Laceration and dislocation of ring finger.

While installing a tramp cylinder on the #2 shorthead crusher, employee sustained a fracture to the fifth metacarpal bone on the right hand, when his hand was pinned between the handrail and tramp cylinder.
Employee stated he was riding in a mantrip and when the trip stopped he could not get out of the bus. He has been diagnosed with degenerative disc disease of the back at L4-L5 and L5-S1.
Member was behind ventilation curtain. Shuttle car was behind miner in the #3 heading. The shuttle car backed out of #3 into crosscut between #2 & #3 entries. The shuttle car caught member between the shuttle car and the rib causing injuries.
Dropping continuous miner cable down from hangers when he felt a pull in his back.
EE, blaster helper, was pulling a 4"" discharge hose, at Pit 548, over right shoulder to get the slack out of the hose & slipped on a rock. His right knee twisted and made a crunch noise. Was able to walk on knee but hurts. There was snow on the ground.
Ee stated that when he bent over to shovel rail for retrieval he felt a sharp pain in lower back to left leg. Did not report until 2/6/07-stated he was worried about job since he just started. (told this to mine supervisor) He has not called us back, we've left message with mother. (Reason report late.)
EE reported approx 3 weeks after alleged date of event, he experienced pain in lower abdomen at the site where he had previous hernias. He stated he & coworkers were changing belt rollers on the mentioned date. Location of pain is identical area where 2 previous surgeries have been performed. The mine is charging this as occupational related illness.
EMPLOYEE WAS WALKING FROM ONE PIECE OF EQUIPMENT TO ANOTHER AND STEPPED ON A PIECE OF FROZEN CLAY WHICH BROKE AND CAUSED THE EMPLOYEE TO FALL.
Coming down the 3/4-O ramp, the weight in the loader bucket caused the loader to light weight on the rear tires. The bottom of the bucket contacted the ground forcefully jolting the operator and he hit the left side of his head on the cab of the loader.
Employee stepped out of the scale room onto some ice covered grating and slipped twisting his ankle as he fell. His leg was bent under his body. He tore ligaments our the left and right side of his right ankle.
Employee was cleaning a forklift with a pressure washer. Accidentally he pointed the washer spray gun towards his left arm causing him an open wound.

Employee was starting up the steps at the RC 4 Tower from the 2nd floor to the 3rd floor. As he reached the 3rd floor he slipped on ice causing him to fall onto his right arm. Wiley was then transported to Boone Memorial Hospital. X-rays were taken and were negative.

Drill operator had gotten off his drill. When he stepped on the bottom step to get back on the drill, the steps moved underneath the catwalk causing him to pull something in the groin area.
Employee was hooking up a welding lead to thaw a water line when he slipped on the ice. *Note* Employee returned to work on 2/5/07. He could not get a doctors app. till 2/22/07. On 2/22/07 the doctor put him on leave.
While trying to remove buildup on rollers, came into contact with moving part and fractured upper right arm.
Employee exiting kiln area when a wooden 4""x4"" snapped allowing bricks to roll off of conveyor and struck employee on arm & back causing bruising.

Employee was tamping when he went to foreman stating this his side was hurting bad and was getting weak. He said he was going to have to go home and go to the doctor next day. He left at 1 am.
The employee was removing a 4' by 8' screen deck - it became stuck - pressure/tension was applied. The deck section came loose and struck the employee on the right ankle when it dropped free.
While servicing a piece of equipment, EE was pulling grease hose and walking backward. He tripped and fell backward onto his back.
