Accident type, without injuries

The continuous miner began cutting the face of #5 entry when a significant sandstone roll was encountered on the initial sump. The miner head contacting the sandstone created a spark, which in turn caused an instantaneous flash, presumably due to methane.

A small rock burst occurred on the 3700 level in the vicinity of the 230 raise bottom at track level. No one was working the area at the time. At approx 11:50 am I was contacted with information regarding this event. The outburst had hit in the hanging wall of the drift affecting 35 to 40 feet of the rib. The 230 stope crew will be doing the clean up work.

A frictional methane ignition occurred on the Gil-6 longwall panel. The ignition occurred at shield No. 19. The flame lasted approximately one minute.

An unplanned connection was made to old workings of an adjacent mine. The workings produced a minimal amount of water with no hazardous gases.

The face in 27KW was shot at the end of the night shift, approx 6:30am. When the powder crew went underground to inspect the faces, they found 5 small fires in the face and muckpile. After checking CO and methane readings, supervisor extinguished the flames. No re-ignition occurred.

Small personnel cage lost power. Large cage was being used to complete repairs to the main shaft. Decision was made to evacuate five (5) underground miners until repairs could be made. Call made to MSHA hotline at 1953 hours.

Employee was operating WT-8 water truck, travelling towards Job #14 at approx 800pm. a 1998 Chevy truck traveling the same direction hit WT-8 in the right rear area demolishing the pickup truck. The driver, though bleeding heavily, left the scene on foot. 911 call was made by foreman & after searching for injured driver (to no avail), MSHA & DMME were called.

An unintentional roof fall occurred at entry #1 at 5+40 station on the 9th West Panel at the #1 seal at the 2A north seals. The fall did not damage the seal itself, affect ventilation, passege of men or cause pesonal injury. The fall measured 30 feet x 25 feet x 9 feet thick. The fall will be cleaned up enough to make the examination of the seal.

During a weekly airway examination a roof fall was discovered approximately 40 feet inby spad no. 967. The fall measured 40 ft X 20 ft and above the anchor zone using 48 inch double lock roof bolts. The fall is 12 cross cuts outby the face. The fall area was supported and dangered off on all approaches. The fall will not be cleaned up.

A roof fall was found on a pre-shift exam. of the 4-1/2 in-active section. The fall was at the 2nd intersection of the #8 room of the #5 entry. Fall is approx. 20'L. by 30'W. and above the anchor point of the 6' resin bolts. Cause of this fall was clay veins & slickenslide roof. Compliance with rules/regs not a factor.

Roof fall occurred in intersection of #1 entry and crosscut #13. Material fell out to the slicken side top just above the anchorage. Fall measured 38'L x 18'W x 4'H. Area had been bolted with 4' resin bolts. No injuries or equipment damage.

The mine experienced a roof fall at S.S. 1+50, entry #6 of the 1st East Panel, off the 1st North Panel. The fall measured 40' X 19.5' X 7', and occurred in a 4 way intersection. Fully grouted 6' roof bolts had been used as roof support, the area was developed on 7/6/2007.

During a weekly airway examination a roof fall was discovered in #4 entry 30 feet inby spad # 937 and extended 30 feet inby spad # 951. The fall is 20 feet wide approximately 40 feet long and 5 feet to 6 feet high. It is located approximately 20 cross-cuts outby the face and will not be cleaned up. Approaches are timbered and dangered off.

A roof fall 18' wide x 30' long x 8' - 10' high was found in an outby area in 9 main rooms #3 6 panel 3 crosscuts inby station 4264 in crosscut 3x4.

An unintentional roof fall occurred at entry #9 at station 7+20 on the 5B panel (1st South off of the 8th West). The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 30 feet x 25 feet x 6 feet thick. The roof had been supported with 6 foot fully grouted roof bolts.

A 3-Way Intersection fell 8' - 9' high by 18' long by 20' wide. the area was bolted with 5/8 x 72"" fully grouted rebar with 6 x 6 plates and 5' x 13' straps. The area had been inspected 2 hours before the fall. It started to bow and work. The foreman reported it at 4 pm. It was dangered off and cribbed. The fall will not be cleaned up. Bad geology was the apparent cause.

An unintentional fall of roof occurred on the 2-B conveyor belt between cross cut # 22 and 23 on the 4th South Sub Main, off the 5th West Parallel. The fall was approximately 30' long x 18' wide x 6' thick. The fall appeared to be loose stack rock that was supported by 6' point anchor roof bolts. The fall will be cleaned up.

An unintentional roof fall occurred at Crosscuts 14-15 of the Flanningan North Mains 1640' to 1727'. The fall was approx 100', approx 16'-17' wide, and approx 8'-9' thick. The fall did not affect ventilation or passage of personnel. The fall will not be cleaned up.

An unplanned roof fall occurred in the #2 entry of 2 Left, 2 x-cuts inby survey station #325. Fall was approximately 19' wide, 24' long, and 4' plus high. Area was bolted with 4' tension re-bar bolts with 5 rows across per the approved Roof Control Plan. Area was timbered and dangered off. This did affect the travelway.

A roof fall occurred at 1st North, Entry #7 between Crosscut 68 and 69. The fall measured approximately 100' long x 16' wide x greater than 9' thick. The fall did not result in any damage to equipment or property.

A roof fall was discovered in the 2 West 1 Mains, 83 crosscut, #7 entry. The fall measures approximately 17 feet long, 17 feet wide, and 6 1/2 feet thick. The area is supported with 5 foot fully grouted resin bolts.

A roof fall occurred at mainline 17-04 crosscut, track entry to return, ss #470+48. Mined 06-2003. Approx 16' - 18' high, 15.5' wide, 51' long. Cover 970' bottom of coal 2+53. Surface elev 1230.

Incident occurred on a stockpile at the power plant. While pushing coal to a feeder, a D11 dozer became stuck over one of the feeders. The operator was instructed to stay in the cab until coal could be removed from around it, this took approximately an hour. MSHA jurisdiction for this event is under dispute.

During an MSHA quarterly exam of the Hart St. elevator a malfunction occurred. They were in the process of testing the overspeed brakes, with the elevator traveling at full speed. When the brakes were set this caused one of the five elevator cables to come out of the sheave on top of the elevator cage.

A roof fall occurred in the #6 Intake entry at survey station #2317, approximately 300' out of the face area. The fall measured approximately 30' long X 30' wide X 5' thick. The area had been bolted with 42"" fully grouted roof bolts. The roof in the area was a laminated sandy shale.

The slope car was pushed back on the bottom which set parking brakes. No one realized the brakes had been set which drained the battery. Battery was replaced but contact tips were stuck. Circuit would not clear so the contacts had to be replaced before car could be used. The car was back in operation at 11:15 pm on 8-28-07.

An unintentional roof fall occurred in the #3 entry of 4 South Mains, the fall occurred inby 61 x-cut. The fall measured 16ft wide by 12ft high by 60 ft long. ***No personal injury or equuipment damage

The #8 elevator went down from 8:18 am until 10:35 am. Landing switches at the top & bottom were adjusted. The elevator was then reinspected by the electrictions and by the PA dept. It passed inspection and was back in service at 10:35 am.

An unintentional roof fall occurred approximately 300' inby SS# 1304 in left return. Fall dimensions were approximately 4'H x 18'W x 60' long.

While moving materials with the 3.5 LHD, the LHD struck the proximity switch to the outer doors of the hoist. The hoist was inoperable for more than 30 minutes for repairs. Repairs were completed at 2:00 a.m. on 8/29/07.

During the preshift for the first shift back after an idle weekend, the fall in south ""B"" #9 entry #20 xcut intersection was discovered. This is an intake entry (not the escapeway) 3-way intersection, bolted with 5' fully grouted roof bolts. Fall was just above anchorage and confined to the intersection 18x13x5 feet.

Employee was performing pre-operational check when he found that the hoist was not operating correctly. Hoist was down due to a bad amplifier card.

A roof fall was discovered in the 2 North 3 Mains, Crosscut 17, #2 Entry. The fall measures approx 20' long, 17' wide and 7' high. The area is supported with 5' fully grouted resin bolts.

A roof fall occurred in the #11 seam, in the 1 north, just inby crosscut #38 and ending in the pillar line between crosscut #39 and #40. The fall measured approximately 80'x18'x6'. The area was supported with 30"" and 42"" fully grouted bolts. An undetected slip was determined to be the cause of the fall.

Rock fall occurred on 1 North Mains at break 20 in the #7 entry. Fall measured approx. 15' wide x 60' long x 6""-5' thick.

Service cage arrived at mine level and kicked out with warnings, ""Mine level magnetic switch,"" and, ""Mine level e-stop."" The mine was evacuated by the production hoist. The underground magnetic switch was changed out and miners were returned to the mine at 8:00 am.

An examiner found a roof fall in the 3 left 4 West worked out panel while on his examination route. The fall was located at crosscut 40 in entry #4. The cause of the fall is weak laminated shale commonly referred to as stack rock.

A roof fall occurred on the main north track, outby the 24 east track south at 193+00 feet @ 4 entry. The fall was 20 ft wide, 25 ft long and 6 ft high. The fall occurred due to the deterioration of the immediate roof around existing bolts. Area was mined in June 1997. Moisture in air caused deterioration between roof bolts and ribline.

An unintentional fall of roof occurred on the 1st Main West number 4 entry at cross cut number 13. The fall was 20' long x 20' wide x 25' thick. It occurred in a 4-way intersection in the supply road. The area was supported by 10' point anchor roof bolts and steel rails. The fall appeared to be loose stack rock. The fall will be cleaned up.

The Somerset Portal elevator stopped operation when a linkage arm for the bottom gate elevator doors came off the switch. The elevator service contractor was called, repairs were made, and the elevator was put back in safe operation at 9:00AM. There were no persons injured, or trapped as a result of this incident.

An unplanned roof fall occurred in the #2 Gas mine 50' inby spad 110 in the #3 entry return aircourse. This is a continuation of a fall that happened on 7/12/07. No injuries were reported. This area was developed on 7/12/07.

On Sunday August 26, 2007 a fire started near the county road close to the crusher/conveyor entrance and burned onto the mine property. Three to Four acres burned and the fire was extinguished in just over the 30 minute time period. The likely cause was a cigarette butt.

A roof fall occurred in the #11 seam, in the 2 main north, #1 entry, from crosscut #175 to # 177. The fall measured approximately 160'x19'x6'-10'. The area was supported with 30"" fully grouted bolts. An undetected slip was determined to be the cause of the fall.

The control office was struck by lightning during off shift hours. The plant was not in operation at the time of accident. The plant was not scheduled to operate on the weekend thus no persons were at the control office during the time of accident. Fire was discovered and 911 call placed. Local fire department extinguished.

An unintentional fall occurred at #35 x/cut in #5 entry of the 3rd north panel at the 21+80'. Fall occurred due to adverse roof conditions above bolt anchorage. 6' tension rebar bolts were used. Fall was 19' wide by 30' long by approximately 7' high.

A roof fall occurred on the Main South roadway between crosscuts 43 and 44. The cause of the fall was a deteriorating shale roof. The fall is being cleaned up and the area is being re-supported with longer bolts.

a roof fall was found on 8/24/07 at 11:00 am at cross cut 93 entry #4. Fall was in four way intersection that was mined sept. 8 1991. Area had been supported with 6' full resin pins. No injury resulted. There are currently no plans to clean fall up.

Following a cave in the gob behind the longwall face elevated levels of CO were detected at the tailgate end of the longwall face. The Mine Evacuation Plan was implemented, all underground personnel evacuated. Management personnel immediately investigated the incident. Less than 5 PPM CO was detected along the longwall face, return and bleeder entries of the affected area.

A roof fall occurred in the 1 main east, crosscut #56, intersection of #3 entry. The fall measured approximately 20'x20'x6'-7'. The area was supported with 6' tension rebar bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.

Due to the Joy 14-15 Continuous Miner loosing power in the last lift of a pillar block in the #2 entry, a roof fall (40' X 40' X 5'-7') fell on it. No injuries occurred and the Continuous Miner was recovered.

An unplanned roof fall at and above anchorage of original 48"" fully grouted roof bolts. The fall was discovered on a weekly exaim and was 15000 ft from any active workings. It was in neautral area of the old ruby fan entry. This fall was not immediately reportable.

An unplanned roof fall occurred in the #2 Gas mine between entries 1 and 2, 1 break outby spad # 131 that measured 50' long, 8' high and 19' in width. The area was developed 6/27/07. No injuries were reported.

A hoist malfunction occurred which allowed the empty muck bucket to fall to the bottom of the shaft.

A small ignition of gas was experienced at 1:15 P.M. on 8-21-07. The incident occurred in the face of #1 entry on MMU001-1. Rock obstruction was contacted by miner creating a spark which ignited a bleeder at base of rock obstruction. A flame of 3' in length & 6"" in diameter came from the face. The miner was at the 20' mark of a 40' cut. MSHA Inspector came to investigate

On 8/21/2007 a roof fall was discovered by an examiner conducting his weekly exam. The fall occurred at S.S. 13+90 to 16+00 in entry #1, of the 5th Southwest. The fall measured 210' X 20' X 10'.

A sensor showed the pinion brake on the #1 hoist to have a wear switch and brake release fault. The mine evacuation was complete at 7:30 a.m. The hoist was repaired and tested. Miners returned underground at 8:05 a.m.

A horseback fault runs across the section. It had been cut down when it was seen. The intersection in #1 entry failed. 18' x 30' x 7'. The area was cribbed and dangered off.

It was reported that there was a slight depression in the breastworks of the #8 Sediment Pond for the Prep Plant. Heavy rainfall has contributed to this condition. At 5:40 AM the #1 service hoist experienced an electrical problem which caused the hoist to trip out. After investigating the cause of the electrical problem, the miners were brought out utilizing the #2 skip hoist. The fault interfered with the use of the #1 service hoist for more than 30 minutes. Two escapeways were maintained during this time and MSHA was notified.

The Chippy hoist operator was bringing an empty skip up the shaft at normal operating speed. As he started to set the brake to slow down, the brake system fully engaged, causing the hoist to immediately stop. This caused the wire rope to whip and jump off of a sheave wheel.

While lowering a longwall ranging arm the hoist tripped on over current, setting the brakes and then the hoist ropes slipped on the hoist drum. This gouged the high density polyurethane rope liners, requiring some liners to be replaced and the drum regrooved.

While drilling a roof bolt from the operator side of the fletcher roof bolter the utility man laid his left hand on the drill head. This resulted in his left hand being caught between the drill head and the mine roof. Employee raised up to get a piece of candy from the top of the MBC. His right foot was outside the operating compartment and was caught between the MBC and the belt structure.

They had the rope threaded through the socket and were getting ready to install the wedge, when another employee lost his grip on the rope and it fell out of the socket, striking the injured employee on the right side of his neck and collar bone.

On 4-11-07 employee pulled on bin gate that was slightly frozen straining right shoulder. Treated with physical therapy with no lost time until worker's compensation decision that surgery was needed. Surgery on 08-16-2007.

This was not a reportable injury at the time, first aid only and no lost time. Several weeks later the employee reported pain in his wrist. The doctor said that there was a sliver of something in his wrist and they remove it on September 19th. He was not at work September 19th or 20th. He was released for work on the 21st.

The employee stated he had tied a shieve around a rib bolt and used it to remove the belt drive. He went to unhook the shieve when a rib roll struck him on the right shoulder and right leg. The employee did not start missing work until 8-29-07.

Employee was moving an oxygen acetylene cart. One of the wheels rolled over a small piece of coal and the cart moved in an abrupt manner. Employee strained right shoulder trying to restrain movement. Attending physician gave pain medication and a sling to restrict movement.
