A small area of smouldering coal was discovered on the back side of the primary coal pile. The smouldering coal was dug out and extinguished by 9:23 a.m. the same day.

Found Sun Night on Pre-shift for 3rd shift, roof fall 3 West, #5 entry, x-c 9 in the intake airway, 30'w x 50'l x 12' thick, area in same slip as last fall in #5 entry x-c 6. Area was dangered off.
An unplanned roof fall occurred in the #2 Gas mine in the #3 entry 320' from the face that measured 70' in length, 18' in width and 12' in height. The area was developed 9-15-07. No injuries were reported.
A roof fall occurred in 4 way inter. Fall approx 5'-6'h, 17'w x 40'l. Area bolted with 6' resin bolts & 6"" x 12"" hole boards. Prior fall area was supported with 4' wooden cribs. Fall is at edge of pittsburgh piller & solid coal area. Called hotline. Over 240' top shale laminated. Sub main #3 1 brk 2blocks outby spad #419. Fall observed
6 Mains 2, 7 block, #5 entry, 4-way intersection, MMU 007. 17 ft. wide x 30 ft. long x 7 ft. high. Support: 6 ft. 3/4 inch resin bolts, pans, straps, 12 ft cable bolts. Fall dangered off, additional roof support installed around fall. Fall clean-up plan developed and posted at fall.
An unintentional roof fall occurred in the 4th SE submain #5 entry spad # 30+06 and this is an idle area. Size of fall was 25' long X 20' wide X 20' high. Type of roof was shale.
001 section cross cut between 1 & 2 entry 2 breaks inby spad # 4146 # 2 entry. The fall fell between #1 & #2 entry last open break. Length Approx 100' x Width 19' x Height 4-5 foot. The fall will be cleaned up.

An unintentional roof fall occurred in the 11 H section at SS# 122+65 in the #4 entry. The fall measured 12' high x 16' wide x 40' long. *** no injury in result of fall.

THERE WAS A FIRE IN THE ELECTRICAL CABLE TRAY LOCATED ON THE BURNER FLOOR. AN ISOLATION VALVE LOCATED ABOVE THE CABLE TRAY LEAKED ALLOWING COAL DUST TO ACCUMULATE ON THE CABLE TRAY. IT IS BELIEVED THE COAL WAS IGNITED BY HIGH TEMPERATURE ON THE COOLER BAG HOUSE DUCT DURING A PUSH. THE FIRE WAS EXTINGISHED. LOCAL FIRE DEPARTMENT WAS NOT CALLED TO THE SCENE.
Small roof fall located at #1 entry and #23 break on the headgate in the return. Size is 8 feet high by 10 X 10. This happened between Sept. 26 and 10:00 am today Sept. 28th, 2007.
A roof fall was discovered in the #5 entry of #1 northwest about 50' inby survey station #148. The fall measured approx 20' long X 10' wide X 5' thick. There were no injuries. Ventilation and passage of miners was not affected. The fall will not be cleaned up. There was no equipment involved.
A power line contractor came by the maintenance planner's office and told him he noticed a gas pipeline leak near the auxillary stockpile area. MSHA was immediately called and the haul road near the leak was blocked to thru traffic. The pipeline was closed in and the pressure bled down. The haul road was re-opened at approx. 1:30 pm. Repairs completed at 6:00 pm 9/28/07.
Roof fall in connecting crosscut between track & belt at 17-7 main line. 1008' cover; mined 8/7/03.
Fall was 80'L - 20'W - 5'H. No injuries and the passage was not blocked. Airway was not blocked. A clean-up plan was posted and all access points were breakered off. Fall was in the belt entry at #24 break #2 section spad #21477.
A rock fall was discovered near end of shift on 9-27-07. The fall was located 7 crosscuts inby the portal. The fall was approx. 30' long x 20' wide by 0'-3' high. The fall was located in the return aircourse.
A roof fall was found at 7:10 AM by employee looking for water stoppage. Fall had broken water line. Fall was in four way intersection at cross-cut 172 main south, entry #2. Fall was approx. 10' high, 20' wide, 30' long. Area was mined January 1997, and supported with 8' full resin bolts. No injury resulted and fall will not be cleaned up.
An unplanned roof fall occurred in the #2 Gas mine that measured 80' in length, 20' in width and 12' in height. The area was developed 9-9-07. No injuries were reported.
The #8 portal elevator went down approx 4:30 PM. A bad RO coil was the problem, which was replaced. The elevator was tested and returned to service at approx 5:32 PM.
A roof fall occurred in the 2nd West, entry #9, xcut 35. The fall was 20' long, 20' wide & 8' high. The area was bolted with 6 & 8' point anchor bolts. The fall did not affect ventilation, block travelway or cause injury. No one is required to travel or work in this area.
A rock fall in old works area was found, it's location is Hog Mains, #5 entry, break 22. Ventilation was not effected and no persons were involved. This area was cribbed and dangered off.
A small instantaneous flash occurred when the miner was cutting in 2W entry face.
Unintentional roof fall discovered on the above date during weekly exam. Fall located at West Mains, 9 Break, Track entry, 2 blocks inby spad 4705. Fall measured approximately 30'LX40'WX 10'H. Area previously supported with 6' full grout resin and cable bolts. Area was developed on or about 7/20/06.
A roof fall occurred in the #9 seam, in the 18 north, #10 entry, just inby the intersection of crossuct #21 and ending just outby the intersection of crosscut #39. The fall measured approximately 970' x 18' x 5' The area was supported by 5' fully grouted roof bolts and 9' mega bolts. An undetected slip was found to be the cause of the fall.
On Sept 24, 2007 a roof fall was found near spad #739 along #7 belt in the #7 entry. The fall measured approx. 74' long x 20' wide x 7' thick. The fall does not impede airways or travel and will be timbered off to prevent travel.

5 men trapped for 18 minutes as elevator was ascending. The governor switch on the miracle run elevator was replaced.
A roof fall occurred in the # 11 seam, 1 m. east, cross cut # 30 in the intersection of # 7 entry. The fall measured approximately 18'x18'x4'. The area was supported with 30"" fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

On 9/25/07 an unintentional fall of roof occurred at cross cut #32 in #4 enrty on the 4th South Sub Main. The fall was 18"" Long x 18 wide X 10' thick, and will be cleaned up. The fall is fractured laminated rock.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE MINE EXAMINER IN THE 2ND WEST MAINS, #6 ENTRY, BETWEEN X-C #9 AND XC# 10. THE FALL MEASURED APPROXIMATELY 60' L x 18' W x 4' TO 5' H. THE AREA WAS DANGERED OFF AND ADDITIONAL FLOOR TO ROOF SUPPORT WAS INSTALLED.
The Hart Street elevator failed to operate properly. The elevator stopped approximately 50 feet from the shaft bottom. The elevator stopped and set the brakes when the governor rope sensor read an overspeed condition. The elevator was reset and returned to service at around 8:30 pm.
A roof fall was discovered in the 2 west 1 mains, 59 crosscut from #1 to #2 entry. Fall measures approximately 80' long, 18' wide and 10' thick. The area is supported with 5' fully grouted resin bolts.
An unintentional roof fall occurred at the 5th North Headgate Entry #1 at 3321 North, Crosscut 44. The fall was of unknown length, width and thickness. The fall did not affect ventilation. The fall will not be cleaned up.
A roof fall was discovered at break 43 on 3a belt line. The estimated size of the roof fall is approx. 6' high by 20' wide and 25' long. no injury where reported as a result of this roof fall.
The Camp Run Portal elevator was inoperative from 8:25 PM until 9:25 PM when the govenor rope tripped a pit switch. No damage was done to the elevator. The governor rope pulley was relocated to alleviate the nuisance tripping.
One of the trailing ropes on the skip broke and damaged the other two ropes. The loose rope may have damaged a guide rail beyond the travel range of the skips. The damaged guide was removed, all trailing ropes replaced, tail rope switch repaired, loose guide tightened, structured inspected before release for operation.
A roof fall occurred on 33M track haulage at #80 crosscut spad #64+97. No persons were injured or equipment damaged as a result of this occurrance.
An unplanned roof fall occurred on #1 section #6 entry at spad #17963. Fall measured 45' length, 18' width and 6' height. Fall was inby section dumping point and covered up the Fletcher roof bolting machine, bolter was removed and all entrances have been breakered off.
Mine examiner found roof fall at break #22 in #1 entry on the mains. While doing his preshift. Fall measured 35'l x 19'w x 5'h. Area had been bolted with 4' resin bolts. No injuries or equipment damamge.
Pillars were being mined on the b-left off 2 left off no. 1 north panel. a lift was being mined from the outby end of the block from the 3 right crosscut (1 break inby survey station 184) when a rock fell catching the head of the continuous mining machine due to the soft mine floor the machine could not back out of the place, a retriever was used to free the machine.
An unplanned rock fall occurred in the belt entry #6 xc of the main north #2. The belt was damaged. No injuries occurred. The rock fall was reported to the on site MSHA inspector at the time of occurrence.
Had a fire start in mill control room that lasted a short time. We installed a new control room. Fed the power from a different transformer bank, and ran new power supply.
Some electrical wiring had gotten together and got hot enough to burst into flame, burning some insulation off of wiring and rubber off of hyd. hoses. The fire was put out in less than 10 min. A call was made to MSHA within 10 minutes of discovery. There were no injuries.
While traveling the No.7 entry, a horse back type roof fall was discovered, approximately 50 feet outby spad No. 399. The fall was approximately 5 feet hight, 10 feet wide and 20 feet long. 48"" fully grouted bolts were used. The area is cribbed off and will not be cleaned up.
The mine encountered a roof fall at S.S. 8+70 entry #4 (belt) on the 5th Southwest. The fall measured 20' X 19' X 8'. The area was developed on 5/1/2007 with 8' point anchor bolts installed. The fall will be cleaned up.

During a fault crossing, a large rock slid off the top of the last shield and obstructed the travelway off the face into the Tailgate entry. The entry remained open.

Bad fuses (2400v at sub station) Evacuation of the mine started at 10:27 am, and was completed at 10:37 am. The fuse were replaced around 11:15 am, and the miners returned underground at 11:20 am.
A roof fall occurred in the 2nd South submain entry #3 at 2170 footage station. The fall was 20 ft long, 20 ft wide, & 8 ft tall. The area was bolted with 6' & 8' point anchor bolts. The fall did not affect ventilation, block travelway or cause injury. No one is required to travel or work in this entry.

Our weekly examiners found a roof fall in a worked out panel in the 1 left - 2right, 1 left, 4 West panel. The fall was located in the old belt entry between crosscut 25 and 26. The examiners noticed the fall as they traveled in the entry adjacent to the fall.

There was a forrest fire that spread onto our property near the impoundment area.
On 9/19/2007 a roof fall was discovered at S.S. 6+40 of Entry #1 of the Main North. The fall measured 40' X 18.5' X 8'. The area was developed on 12/16/2001, with 5' point anchor, and (2) 12' point anchor bolts used as support. This fall will not be cleaned up.
An unplanned roof fall was found at approximately 10:30A.M. in the #2 entry of 1st Left, 1 x-cut outby survey station #81. Fall was approximately 4' high, 19' wide and 10'-12' long. Area was bolted with 36"" fully grouted resin bolts according to the approved roof control plan. Area was timbered and dangered off. Did not affect any equipment or ventilation.
Fall discovered which occurred between 9/17/07 and 9/19/07 on the 001 section in entry #7 spad #19508. Fall was 25 ft x 19 ft x 10 ft high. Both OMSL and MSHA were notified. Area breakered off, will be cleaned and rehabilitated. Ventilation was not obstructed.
Unintentional roof fall 6EP Panel #8 Entry footage 5,200 Feet. 20 ft long x 20 ft wide x 8 ft high. 4 Way Intersection. Bolted with 8 ft TR Bolts. Approx date of developement 7/17/07. This is a non-working section, affected no equipment or caused injury. Escapeway was not affected.
An unintentional roof fall occurred at break #58 in an intersection. The fall measured approximately 25 feet long X 25 feet wide X 4 1/2 to 5 feet high.
A non-injury roof fall occurred in the 11 west longwall tailgate. The fall was between the face and the cribs in the #3 entry of 10 west. The fall was 5' wide, 60' long and 4' high. The walkway was blocked.
Rock fall occurred on 1 East Mains #4 belt at break 51. Fall measured approx. 20' wide x 4' long x 3.5'-4' thick.
During an MSHA inspection of our emergency hoist, it was found that the hoist was not functioning properly. The hoist would not release the brake to lower the hoist into the intake shaft. The investigation revealed that the 90 VDC power supply had failed which powered the drum brake release circuit. The power supply was replaced and the hoist was returned to service.
A roof fall occurred on the longwall tailgate entry. The fall was approx. 30' kibg x 18' wide x 6' high. The longwall blockage plan was followed and the fall will be mine through.

A methane gas ignition occurred during mining in the No. 2 entry on the No. 1 section. The miner had cut approximately 20 to 25 feet out of the face, as the miner was trimming the roof on the exhaust curtain side the ignition occured.

Unintentional roof fall occurred on the Main North Track at 20 xc. The fall was 73' wide x 28' long x 8' high. Into the crosscuts. Fall occurred due to the deterioration of the immediate roof.

On a general inspection of the emergency escape capsule, the capsule failed to operate properly. The mine was evacuated until repairs were made and a test was conducted. Mine reopened 4:00 pm.

On 9/17/2007, an unintentional fall of roof occurred in # 5 entry at cross cut # 28 on the 2nd West Sub Main off the 3rd South Sub Main. The fall was approxmiately 22' long x 18' wide x 6' thick. The area was bolted with 6' & 10' point anchor bolts. The fall was laminated stack rock and will not be cleaned up.
Plant was starting up. There was trouble with the indirect firing system and had to go back down. The feeder would not turn. Management was informed at 7:30 am - checked. There was high heat in the vessel. Took all safety precautions - isolated, inerted with Co2 & nitrogen, used heat gun. Temperature went down. MSHA called at 7:45 am.
The Otis elevator was inoperative for approximately 2 hrs due to the door at the shaft bottom being open. This was reported to on-site MSHA inspector. No entrapment occurred. The doors were closed and normal operations resumed.

An unplanned roof fall occurred at parallel mains #3 Entry 1 crosscut inby spad #673. No injuries reported and no equipment involved. Fall measured 200' length, 18' width and 4' to 5' height. Fall was in the intake escapeway. Production was stopped unti escapeway was made passable. Area has been rehabilitated.
An unintentional roof fall was discovered at 5:30 am on Monday, September 17, 2007. The fall occurred at Spad # 2865, 2500 feet outby the working section. The fall measured approximately 20 feet wide x 70 feet long x 5 feet high. Additional support has been added to this area to prevent future roof falls.

A roof fall was discovered in the return on 4Lt. section about 3000' outby near spad #8034 this area was developed approx. 1/10/07. The fall occurred in the #1 & #2 entries and is approx. 4' thick X 20'W by 40'L. The area was bolted with 4' resin bolts. It has been dangered off and cribs set.
On 9/15/07 an unintentional fall of rock occurred on the 5th South Sub Mains off the 5th West Parallels in the #3 entry. The fall was approximately 60' long x 17' wide and 6' thick. The fall was bolted with 6' point anchor bolts. The fall will not be cleaned up. The fall was laminated stack rock.
Equipment operator started to straighten his area up to level out excavator. He noticed a spray of oil and then the rear of the excavator was on fire. The operator evacuated the equipment and moved to a safe area. Local fire dept. called to exting. flames. The excav. was on fire for about 40 mins..
A potential inundation of water occurred as the miner cut into a bleeder area of the mine to establish additional bleeder entries. Water came on to the section and after a while created a possible inundation.
An examiner found a roof fall in the worked out panel of the 3 Left 4 West at crosscut 20 in entry #4. The fall was located in the old belt line of the worked out panel.
A fall of the roof 40' long, 24' wide and 16' to 20' high occurred in the No. 2 heading covering the Joy 14-10 continuous miner JM505 on the 001 section 80' inby SS# 1847.
An examiner found a roof fall adjacent to the travelway along the 5 West roadway at crosscut 33. The fall was located in the crosscut between the roadway and belt line.
A roof fall was found on a preshift at approx 9:00 pm. 1 X cut off of the intake travelway. It covered a complete intersection, it was approx 5' high. 1 Xcut inby station #3030. No escapeway or any ventilation was effected.

While changing out the burner pipe on the discharge of the kiln, an ignition of coal in the preheat tower causing an explosion in the preheat tower. Dust blew out the burner pipe opening.
Main west submain. Approximately 19' x 19' x 6' to 8' high. Blocked supply road, personnel traveled around fall in adjacent entry.

Relay on the main Lower contacts had a broken finger that activates the steps (speeds). The mine was evacuated at 8:10 p.m. the relay was replaced around 1:40 am, & the miners returned underground at 1:55 am.

The fall didn't affect ventilation, cause injury or block travel. It did damage some belt line. No one is required to work or travel in this entry.
Notified by an MSHA employee an unintentional roof fall had occurred approximately 100' inby SS# 3817 in the #1 entry of the abandoned and not active workings 4-Left panel. The fall dimensions were approximately 5' high by 20' wide. Length could not be determined due to area not being accessible.
A roof fall was discovered at S.S. 1+50 of the 1st East Panel, off the 1st South Main. The fall measured 30' X 19.5' X 6', and occured in a 4 way intersection. The fall will not be cleaned up. The area was developed on 7/10/2007, and bolted with 6' fully grouted bolts.
The fall at #7 entry, #23 xc on the south ""B"" submain, continued on riding out the cribs set between the #6 & #7 entry. The fall went through the crosscut from #7 entry to #6 entry and continued to within 15 feet of the #5 entry. The fall was 18' wide, 70' long, 10' high. The area was bolted with 5' fully grouted resin bolts. No one was injured. No equipment was damaged or trapped.
Fall was in #2 entry, 2 section, one break outby spad #21233. Fall was 30' long X 20' wide X 4 1/2' thick, 2000' from face. No injuries or blocked passage to men. Area will not be cleaned up.
Fall in 9 I section at 17 stopping in cross cut 2 to 3 at #41+08.62. Fall was 16 ft. wide, 60 ft. long and 8 1/2ft. to 10 ft. high.

Roof fall at Spad 921, neutral entry outby section #3 about 21 breaks outby face in #2 entry. Roof fall fell across break about 115 ft from edge #3 intersection to break between #1 & 2 entrys. Fall was about 8 ft thick & 20 ft wide. Area was bolted with 6' tension bolts. ""Dangered off Area"". Section is down until area is timbered or cribbed & repairs are made to stopping.
A roof fall has occurred in the main east, No. 6 Entry, Spad 54 + 15. The dimensions of this fall are 20' long, 18' wide and 8' high.
A roof fall was observed by mine examiner in the return rooms of #1 unit. Roof fall locations is in 5th room set-up of 2nd North Main return at spad #16+10 inby to 18+20. Fall is 210 feet long X 18 feet wide X 45 inches high. This fall is in the old works. Fall is timbered around just incase someone needs to go close to the area.

Roof fall in the #2 Entry at spad #2700. Fall was 40' X 20' X 5'. No injuries occurred.
Safety dogs on man cage would not move properly. Called Rolla Office at 3:00 PM. Reported hoist down. Pulled everyone out from underground and MSHA said to call back the next morning. Called MSHA on 9/12/07 AM that hoist was back in service at 1:00 AM.
Storage Trailer was set on fire by unknown vandals. Storage trailer and contents were completely destroyed. No injury or illness ocurred.
A roof fall above the anchorage occurred at 132 X-cut on Main North Track Entry. The area was mined 1993.
At 11:00 AM on 9/10/07 elevated CO reading was detected on monitor located in 6L return. Mine manager went to investigate & at 2:15 PM found hot spot in #000 @ survey station 17+06.24. Access was blocked by roof falls on both sides. We were instructed to load out fall to reach the burning material. Unsuccessful & on 9/11/07 permission was granted to build 3 seals around area.
Unintentional roof fall discovered 9/10/07. Located at 8 block, 4 zone, #7 entry, 1 block inby station 3561. Area previously supported with 6 foot full grout resin bolt. Fall measured approximately 20'WX8'HX8'L. Area was developed on or about 01/31/2005.
The intersection of #7 entry, #23 xc on the south ""B"" sub-main fell in. The fall was 18' wide, 13' long, 10' high. The area was bolted w/5' fully grouted resin bolts. No injuries; no equipment damaged or trapped.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 1ST SOUTH MAINS, #1 ENTRY, BETWEEN CROSS CUTS 4 & 5. THE FALL MEASURED APPROXIMATELY 25' L x 13' W x 6.5' H. ADDITIONAL SUPPORT (TIMBERS) WAS INSTALLED.
A pre-shift examiner found a roof fall during his examination of the 5 West roadway. The fall was located at crosscut 27 between entry 5 and 6, about 12 feet from the edge of the road. The fall was timbered and travel was resumed on the roadway.

An unintentional roof fall occurred at the #4 crosscut, 2-A Supply Travel way road. This area was supported by 10' point anchor roof bolts and the roof fall will be cleaned up and the area properly re-supported.
The number 1 elevator was staying in inspection mode and not working properly at approx. 4:40 pm. It would go down to the bottom and come directly back up. It then started working correctly at approx. 5:45 pm. Schindler was called & cleaned & adjusted the bottom lock. At 10:16 pm it was working correctly.
A roof fall occurred on the 8O section. The location of the fall was in the #2 heading in the intersection located at the #24 break.
During a weekly airway examination a roof fall was discovered in # 1 enty 6 cross-cuts outby the face at spad # 998. The fall has been cribbed off with danger signs and will not requir clean-up. The fall measured 20 feet X 20 feet and 6 to 7 feet high. The intersection showed signs of failure during development and 6 foot double-lock bolts were used in the intersection.
An unintentional roof fall occurred in the intersection of the South East Mains in the # 4 entry, 23xc. The cause of the roof fall was laminated and unconsollidated shale at the anchorage point. The area had 6' and 8' fully grouted rebar roof bolts for support.
Elevator stopped 5' from surface landing for approximately 5 minutes, then continued to landing with no adjusting or attention made to the elevator.
A mine electrician reported an outage of the service hoist. After investigation it was determined that the lower track limit switch was damaged and required replacement. The mine was evacuated and MSHA was notified by telephone at 7:34 p.m. The switch was replaced and miners returned underground at 10:05 p.m.

An unintentional roof fall occurred on the shaft bottom area between the cage area and the man way. The roof strata broke at and above the anchor point.
A rock fall was discovered in the #5 entry 1 crosscut out spad 923 at 2:15 p.m. The fall did not affect miners, equipment nor ventilation. The fall is approximately 9 feet high x 20 feet wide and 90 feet long. This area is well out by the active section. The area has been dangered and timbered off.
Subcontractor hit a gas line with Ditch Witch causing damage to line. Mine site was not inundated with gas due to prevailing winds.

Ware Trucking had a tractor/trailer coal truck to lay over on its side at Mine #5, Charolais Mining Company. No one was hurt; the cab of the truck was o.k. The trailer was bent up a little.

A roof fall aproximately 50'x19'x6' was found at 6:45 pm in the !st. East Sub-main #4 entry 3+50. The area had been mined in June 2007 and supported with 5' fully grouted roof bolts. The fall was caused by a hidden slip.
Our weekly examiners found a roof fall in a worked out area of MMU-004. The fall is located in the 7 Left 2 Right - 1 Left / 4 West panel. The cause of the fall is weak laminated shale. The fall was located adjacent to the examiners route of travel.
An event occurred on the longwall where the top rock fell out of the face and rib on the tail of the longwall to prevent passage of men safely off the tail. The tail blockage procedure was followed.

Roof Fall, 3 west, #5 entry, #5 x-cut, fall was in intake in the intersection in a fault area mined back on 8-6-07. The fall was 50' long x 30' wide x 10' thick.
An examiner found a roof fall during his examinations of the belt system. The fall was located in the 3 West Mains between entries 4 & 5 at cross cut 23. The fall was located adjacent to the roadway and belt line.
During normal weekly examination of return, two roof falls were discovered. Neither of them impeded travel or air movement. They were at the following locations: 1 crosscut outby spad #5525 in K Butt return and in the intersection of spad #8525 in the O Butt return.

The no. 1 elevator went down at approx. 3:50 am. The cable on the return cam wasn't pulling back all of the way. It was adjusted to correct the problem.

A roof fall occurred near the B-west escape capsule. The capsule is no longer in use. The fall was at approximately spad number 31 + 44. The fall was 35 feet long, 18 feet wide and 8 feet high. The fall occurred due to the deterioration of the immediate roof around existing bolts. The area was originally mined in October of 1989.
The continuous miner had cut on the right side of the cut. When the operator started the cutter drum a small flame appeared at the miner head. There was no damage to personnel or equipment.
5:30 PM Monday Sept 3 Rodeo hoist went down. An electrician had been called out for repairs. Doesn't run in auto or manual. MSHA notified at 6:00 PM and told that it was not an emergency, only a reportable. Diane from district called at 6:13 PM to verify that it was not an emergency. Rodeo hoist back up at 2:10 AM Tuesday Sept 4. MSHA notified at 2:11 AM hoist was operational.
A roof fall was discovered on this date and time. The fall was 40' in length, 20 feet in width and approx. 5-6 feet in thickness. The fall was located approx. 70"" inby the #22 crosscut in the #1 entry on A-mains return. No equipment was involved.

Sometime between 3:00 PM 9/1/07 and 9:50 PM 9/3/07, a fall occurred 1 break outby spad # 18097 in #4 entry. The fall measured 35' x 19' x 8' - 10' thick. The area was breakered off and will be cleaned with additional roof support installed. MSHA and OMSL were notified upon discovery.
During a pre-shift mine examination after a holiday week-end a roof fall was discovered in #3 entry, LOB, on 001 section, that measured 60 feet long, 20 feet wide and 6 feet to 7 feet high. The fall was cribbed off and plans are to clean up and re-support the fall.

The mine experienced a premature detonation of a single deck in a single blasthole while performing the necessary checks on the digital detonator system. The blast area was evacuated prior to performing the checks.

Ignition source unknown. Fire occurred in the wet processing section. A slurry pump, electrical wiring & switchgear and the building were damaged. No employees were working. Citizen driving by on Battleground Avenue observed and called 911. Local fire department responded and Plant Manager was notified. Plant Manager called the MSHA Hotline.
On 9/2/07 an unintentional roof fall occurred on the 1-A travel road at cross cut #27 on the 2nd West Sub Main off the 3rd South Sub Main. The fall was 20' long x 20' wide x 20' high. The fall was fractured stack rock bolted with 6' and 10' point anchor roof bolts. The fall will be cleaned up.
An unplanned roof fall occurred in the #2 Gas mine at spad # 78 that measured 25' long, 20' wide and 8' high. No injuries were reported. The area was developed on 3/29/07.
A fall occurred in the 2nd south, #2 entry, 2800 ft station. The fall was 20' long, 20' wide & 8' high. The area was bolted with 6' & 8' pt anchor bolts. The fall did not affect ventilation, cause injury, block travel or damage equipment. No one is required to travel or work in this entry.
A fatal accident occurred when a miner became entangled in a take up pulley on the #13 conveyor belt. A guard was in place at the time of the accident.
A Sein Brute 612 mantrip carrying two miners was traveling at 38-39 xc, 592 S.Longwall Panel 3 Road (1 Room). A mechanical failure within the steering mechanism resulted in the over-turning of the mantrip. A miner was pinned under the mantrip resulting in fatal injuries. The other was ejected, sustaining injuries. The company is investigating this incident with MSHA.
Victim, experienced miner working inside fan structure with another ee using grinder 8-10 ft from secure barrier to protect from shaft area. While grinding, other miner noticed ee missing. After search, his body was discovered at shaft bottom. Incident didn't involve non-compliance w/MSHA rules/regs. Investigation ongoing, but appears victim voluntarily jumped to his death
An electrician that was trouble shooting an installation was electrocuted by a single leg of a 3 phase 4160 volt service.
For unexplained reasons, the miner gained access to a secondary road, adjacent from where he was instructed to do road maintenance. Miner removed berm and came too close to road edge, loosing control of equipment and falling through a steep slope, rolling over with the machine. This resulted in miners death. Miner was not wearing seat belt.
EE was operating continuous miner in the #4 heading was cutting right lift. Approx. 25' when top broke starting at the face falling outby into intersection. Positioned on the right side of the intersection miner from kneeling position located between the 3 & 4th row of bolts outby the face. Head and body trauma resulting in death
While attempting to manually shut the tailgate of his dump truck, He caught finger in tailgate causing amputation of the tip of the index finger of the right hand and slo sraping his middle finger. He was at sight to pick up a load of rock.
The employee was using a stone dressing tool to re-surface a grinder stone wheel. He said the tool caught on the grinder wheel and it jammed against his index finger cutting the tip of it off at the fingernail.
Worker was observing cable used to pull conveyor belt on new conveyor system and became entangled between cable and belt pulley. Fingers amputated on left hand serious injuries to right hand, broken collar bone and broken rib, dislocated knee.
Employee was removing chock block from water truck when rock rolled off highwall & struck left leg. Failure to warn, improper placement and inadqequate warning system, no signs posted for work being performed above highwall.
Employee was helping to connect a rockdust trailer to a battery jeep when he caught his left index finger between the connecting pin and the frame of the jeep. He recieved a crushing injury to the finger causing it to have to be amputated at the first joint the next day.
Employee's hand was caught between hydraulic ram and tube.
The employee was attempting to take up slack on a 12' x 1/2"" wire rope to lift the counter shaft and gear from spiral housing at #19 Mill. His right hand was guiding the rope; his left was giving signals to the crane. The cable twisted pinching his finger between the cables.
EE removed guard, pushed on vee belt to check belt, causing belt to grab and start. This pulled EE's finger into pully and cut off the right index finger at the base of nail.
Injured was helping another ee fix washer and hurt himself, but about 6 months ago, but did not report to anyone. On 9/5/07 injured was helping to put bearings on washer and aggravated.
Was cleaning a mold. Moved latex to fit into a mold. Hand was on edge of mold and got caught between molds which cut finger tip off.
The employee stated he was moving the continuous miner to help free up a roof bolt machine that was hung up when the miner itself got hung up. The employee & 3 other employees were putting blocking under the cat pads & the operator was on the other side of the miner with the remote. The operator let the stab jack up & mashed the EE's finger between the cat pad & the crib block.
EE stated that he had made a push with the rd630 -d10 rock dozer and was backing up and backed over a rock and felt pain to his back. EE refused medical treatment. He did not cease work. On Oct 7,2007, EE called in and stated that he went to the doctor and he had a strained back muscle.
At the time of the incident the tech was working on the front slack adjuster on a CAT 793 haul truck. To access the slack adjuster the tech removed the two front bolts and loosened the two back bolts that hold a step in place above the adjuster. With the bolts out, he tipped the step up and out of the way. As he was working the step dropped down, pinching and fracturing his finger.
While EE was unloading quick lime pneumatically out of a trailer the product hose split spraying lime out. He was hit in the face with the lime. He was wearing safety glasses. Lime went under the glasses into his eye. The cornea of his eyes was scratched and he was required to use eye drops and keep his eyes covered for 48 hours to insure healing.
Employee was assiting a miner by loading bolts into the bolting machine cassette when a rock fell of the rib, striking him in the back. He was pushed forward into the equipment where his left leg impacted the machine causing a contusion, for which the Dr. took him off of work. The heading had been check scaled during the bolting cycle.
Employee had climbed ladder to enter discharge end of kiln. Ladder footing began to slide on IKN grates and caused employee to lose his balance. Employee fell and rolled onto his right side hitting some material and hurting his ribs, head and finger areas.
Pulling klinkers he pulled a muscle between shoulders and neck.
Injured was backing a dozer off of the clean coal stock pile when the emergency brake set causing the dozer to come to a sudden stop. The injured jerk his neck when this happened.
Removing broken can crib from 10 west tailgate walkway. Employee was using a chain and come-along, when the chain broke. Employee was hit by come-along in the lower left side, resulting in a broken rib.
Employee was walking to mantrip, stepped in hole and twisted knee.
Changing a bottle of water in the water cooler. Picked up water bottle, then felt burning and deep pain in groin area.
Helping load rock dust into a scoop bucket, while lifting a bag of dust he felt pain in lower back.
The clay bank in pit #1 slid into the coal pit, shoving an end dump truck which slid into the loader. Five inches of rain, 36 hours before the slide, was a contributing factor.
Employee was lifting a piece of 6"" pipe and as he picked it up to move it out of the walkway, he felt something pull in his left groin.
Injured claims to have been lifting a piece of metal 3/8"" x 12"" x 36"" long during clean-up when he felt pain in lower back area. Injured threw piece of metal in end loader bucket and started driving 773 rock truck for approximately 45 min. when pain got worse and he called for help. Supervisor was notified and ambulance called.
Employee was lifting rail track when he felt a pinch in his left side. He was taken to the doctor and received prescription medication.
Member was dismounting a D-10R dozer when he slipped and fell.
Employee's ankle was caught between the input shaft and pressure plate attempting to change a torque converter on rail locomotive in the yard. Cause of this injury was safety awarness and having help. Compliance with rules/regs not a factor, miner proficiency a factor, PPE not a factor, mining equip/systems could be a factor.
During over the normal use of the dozer over a period of years, employee claims that his back has become so painful that he is going to the doctor.
It was dark outside. He turned to come down the steps of the coal truck. He lost his footing and tried to grab the rail. As he fell his left leg hit the steps. He landed on his back and left shoulder.
Worker was removing a 3/8"" pipe fitting from a solenoid valve when he felt a pop in his shoulder.
Employee was trying to hoist himself up to look at screen deck which had been shut down but was still slowing down. He stepped on the guarding of shaker shaft. The rubber part of guarding gave when stepped on and his left foot went through to the shaft which had not come to complete stop yet. The turning shaft pinched his foot causing a cut on the bottom without cutting his boot.
Employee was in the process of turning on water hose at #6 belt drive and as he turned valve on from the hard line going to the hose he was using it jerked his right arm. Employee was diagnosed with a torn bicep tendon and repaired on 10/11/07.
Putting a pallet in a rock car he felt something pull in his left bicep. Injured continued to work his normal job until 1/23/08 when he began losing time for left bicep repair surgery.
Working on belt with slate bar. Picking up structure, when a piece of rock fell striking person on left side, knocking him down, catching both feet under rock.
While EE was stepping down on access ladder for a screening unit, he slipped, and injured shoulder and hand.
Replacing park brake on rubber tire dozer. Park brake got wedged into the center of the machine and then came loose. Brake sprung up and smashed index finger of right hand between park brake assembly and main frame of the machine.
Employee stated that while operating the dozer the segments slipped in the track causing the dozer to jar him.
Employee was in the #4 entry helping the miner operator move the continuous miner when he stepped into a hole and turned his left ankle. This section had moved to a new mining area and was experiencing bad bottom conditions.
EE was backing down a ramp in a loader and ran over wet material that had slid across his road. The impact jarred his back and that caused a back strain.
While operating a trolley jeep the pole came off the wire. The employee attempted to put pole back on wire and struck head against the roof. Employee started missing time on 9/29/07.
The employee stated that he was operating the scoop cleaning the roadways when the scoop hit a rib board and caused the board to strike his head.
Repetition of lifting and bending for stone.
EE tripped and fell into rib, caught weight on right hand/arm.
Employee was setting blocks under hoist while setting it on hoist pad. Hoist set down on employees right thumb, causing pinch/mash.
Employee was backing up a haul truck and ran into the dipper of a shovel. Employee was jarred in the truck.
Worker had just lifted the 25hp motor a couple of inches off newly installed motor mount so he could re-adjust mount. The motor moved over to plumb itself causing clamp overhead to slip off, this allowed motor to fall strking mount, then fall hitting trailer axle, then striking EE's thigh before coming to rest on ground.
While backing up 4838 cart, looking over his shoulder - heel of left foot caught on pipe handle attached to bandsaw twisting his ankle. Seen by physician and off of work.
MVA - Driving unit # 88103 shot loader down a haul road to bins, made a turn, truck flipped making the turn, hurt shoulder.
EE was operating the water truck on the haul road. He started up the ramp entrance to the pit area, as he got to the top of the hill, his truck started spinning on the wet road. The truck slip ped backwards, down the ramp and hit the berm, which caused the truck to turn over. There was cab damage to the truck.
While driving T16 truck (240 ton) the operator drove on to the left hand berm causing the truck to tip onto it's right hand side. Employee was taken off of work per physician.
Employee was carrying a post along the beltline. He slipped and fell on his right knee. He started missing time on 9/27/07.
Injured person was in dump truck being loaded. Loader operator loaded truck too quickly and jolt to truck snapped driver's head to the side, injuring his neck.
Employee was shoveling in muddy area (below conveyor #3), slipped and fell back on his right side. sprained/strained cervical.
Employee stated that while operating front end loader he experienced back pain
Employee was running centers in the No. 3 entry. The ripperhead on the miner needed let down. Employee was standing behind the miner using the remote box to drop the head. As a result the miner rocked back, setting down on his left foot.
Too close to forklift; it ran over right foot breaking ankle.
The employee slipped on a rock and twisted his right knee on 9/26/07. He continued working until 12/19/07 when he visited a surgeon that recommended time away from work.
Employee bent over to pick up something and could not straighten up. Diagnosis is back spasms.
DID NOT START LOSING TIME UNTIL 1/17/08. While pulling on conduit on miner cable, strained left shoulder. Surgery required on 1/17/08.
EE out-by at #6 head drive working on scoop. Draw-rock fell from between bolt and rib onto low back. Taken to ER for treatment. X-ray negative, doctor instructed him to take remainder of week of (2 days).
Shuttle car operator got his head between roof of the mine and car, causing light concusion and tissue tear.
EE was getting out of roof bolting machine, slipped and strained lower back.
While cleaning debris from the grizzly feeder he lost his balance and slid down a chute about 39"" onto a pile of rock. One of the rocks rolled down and bruised his right leg. This machine was shut down and locked out at the time the task was being performed.
Employee was walking to mantrip and stepped on a rock, twisting his right ankle.
Employee has recurrent low back pain after handling material in the mine. He was diagnosed with a lumbar sprain and started missing time on 9/826/07.
Mechanic was loading track pin into back of service truck when he dropped it on his left foot.
The scraper started to proceed down an incline following the curvature of the road when the tail-end began to skid towards the outer edge of road. The scraper bowl contacted the ground and the push block also nosed into the road, causing the scraper to roll several times down an incline. Operator escaped through the rear cab window after unsecuring his seat belt.
Member was dismounting the D-9 Dozer when he stepped down from track, his foot slipped & he caught himself with left arm, causing strain to his arm & neck. Went to ER Room and was told to see specialist. We are reporting because of 10 day policy This is information we have at this time.
Employee reported: He and co worker were to bolt the face of the no. 6 entry. When he was inserting the bolt in the drilled hole, draw rock fell (4'x2'x5"") between the ATRS and machine's canopy. The edge of the rock struck EE knocking him to the mine floor.
Checking battery charges in surface shop, he tripped over charger cable and fell onto the floor and SCSR on belt.
While stacking pallets, employee felt pain in upper back.
Employee was unloading core tube on core drill. Using a hammer to dislodge the core sample a large piece of rock came loose and dropped on employee's left ring finger.
EMPLOYEE WAS HANGING CABLE WHEN HE STRAINED HIS BACK.
Employee was flipping the gate on S-9. After removing the nut on the Waianae side, employee was pulling the bolt out with his right hand and catching the 2 washers with his left hand. The gate fell down, crushing his finger between the handle and angle iron.
EE reported: Working an extra day, assigned job of building a stopping. After completing the stacking process, was to begin applying plaster. When he picked up 35lb bucket of plaster, experienced pain to his low back. Pain worsened, came out of mine at 2:30pm, treated by local physician, removed from work staus til 10/1/07.
Employee was climbing down a two-rung ladder in the Morenci Concentrator and felt his left knee buckle and bend backwards.
EE was helping to change bits on the continuous miner. Another employee had been tapping a bit with a hammer and EE thought that he was finished hitting it. As he grabbed hold of the bit, the other employee struck him on the right thumb, resulting in his thumb being broken.
Rib coal & rock rolled out struck subject's lower back.
The injured was operating a continuous mining machine in the #2 entry of the rooms mined left off (about 133' inby SS #5867) northwest mains. The rib rolled and a piece of the rib consisting of coal and rock (approx 4"" thick X 9"" wide X 8' long) struck him, resulting in fractured ribs and a fractured pelvis.
Employee had track switch up for repair, when he lowered the jack, it slipped and the bar hit him in the face (broken teeth)
Employee slipped and fell while cleaning a belt line. He fell on his left knee. Employee began missing work time 10/15/07.
WE WILL CONFERENCE THIS INCIDENT: Employee twisted ankle jumping out of pickup truck bed while in process of unloading 4-wheeler during a personal trade before shift started.
While loading belt structure into a parked scoop bucket, employee stepped on a top roller. The roller rotated under the weight of his foot, causing a twisted left ankle.
Employee was leveling belt structure. When he lifted bottom stand he felt a burning sensation in his back. First day missed due to injury was 9/26/07.
The inside of an employee's left knee was hit by a rock that bounced off #3 crusher while he was cleaning with a water hose.
Miner was moving a loader when the engine belts began slipping causing smoke inhalation.
Employee lifted a 118 lb. part from the floor into a parts washer (3 ft above floor). Employee felt a twinge in his back. Worked the rest of the day. Came to work the next morning with severely sprained back. Suffered severe lumbar sprain, possible ruptured disk. Sent for Cat Scan to determine extent of injury
While walking carrying a 48# wt. he felt his ankle give and he fell to the ground.
Employee was climbing up onto a Mack rollback, when he slipped and fell into a low block wall, striking his side and back. Employee fractured L2 and L3. Employee was released from hospital with a back brace.
On Sept. 24, 2007 EE stained his back when he moved a bearing assembly he was hooking a chain hoist up to as it was lying in a pick-up truck bed. He started missing work on March 3, 2008.
Rail was on forks, fell off and fell on his left foot.
Claimed by the employee: He was stepping to the ground from his 777 rock truck. He twisted his ankle and fell to the ground, spraining his ankle. Employee said he was carrying his dinner bucket, therefore he didn't have three points contact. Frasure Creek Mining doesn't think this happened on the job. It wasn't reported until the next day.
Employee was traveling down slope in scoop when brakes failed and scoop struck supply car.
EE was utilizing the top of a battery box on a DBT scoop as a stable platform to support his notebook while he was writing notes. The roof bolt operator was unaware of EE's location when he started tramming the roof bolter forward. The roof bolting machine was trammed approximately 10 feet forward when the bolter struck EE from behind.
Employee was attempting to hook up a coupler to the scoop and the pin that went down thru the hole pinched his finger. Cause of this injury was an unsafe act and safety awareness. Compliance with rules/regs was a factor, protective equip was worn, miner proficiency a factor, mining systems could be a factor.
Miner operator had A & B run out of face of #4 entry on south section. The clean up was in progress. Some rock had fallen out of last 10' of cut & was being loaded out. While cleaning up right side of cut the operator stumbled over a piece of the rock falling on miner remote and hurting ribs/hip/shoulder.
While the green ram car was positioning to dump, EE started across in front of the feeder. The corner of the ram car bed caught his right foot pushing him toward the feeder area. The ram car operator started to dump, not knowing EE was in front of his ram car, when he heard him screaming. The operator backed off of EE's right leg, ankle and foot.
Employee was checking fluid levels on truck #773 (100 ton truck). When he finished he started down the ladder on the truck and pulled a muscle in his back.
The employee laid over 5-6 feet of miner cable when he fell to the ground complaining of severe lower back pain. Employee started missing work 9-25-2007.
While employee was transporting scoop underground scoop tire became tangled in belt roller, which flew up and struck him in the face, causing injury to face and or head.
The injured employee was assisting on a repair of a hydraulically operated machine component, when the hydraulic line came apart. The employee was going for clean-up material when he heard a ""pop"" and felt a sharp pain in the rear of left leg/knee. After seeking medical treatment, he was diagnosed with Tendonitis. He has been released for duty and has returned to work.
Employee stated that while lifting a 5 gallon bucket of material, she felt pain in her arm and shoulder.
Employee was using a pry bar to free up a stuck diverter gate. The ram to the gate had been disconnected. Apparently, the gate became free and slammed against the pry bar pulling his forearm into the opening and crushed his forarm between the edge of the bin and gate. He suffered a broken radius bone above his RT wrist.
Employee was walking backwards to move away from the bolter stepped on rock and felt pain in right foot.
Laceration on left arm caused by contact of sharp end of a metal strap.
EE helping load 2 pcs of cribbing into back of pick-up. 3.5 hrs later, EE claimed he aggravated a pre-existing, non work related back condition.
Operator backed and stopped loader within 3' of kiln door. After operator stopped, EE stepped behind the loader to pick up wire cable and shackle. When he stood up, the loader inexplicably lurched backwards pinning him against the kiln door. The right rear of the loader struck EE's right arm, hip/torso area, compressing him against the door.
Walking from man-trip to face, carrying bucket of roof bits, 2 pieces drill steel, & lunch bucket; slipped in mud, fell onto back. Taken to E.R. for treatment, x-ray negative, told to take off until 9-26 and see family Dr. if necessary.
Employee was putting Socks on Bag House, when he slipped off the rails. He straddled one of the bars which are about 2""x2"" wide. He landed on his scrotum area.
New 3/4"" wire rope cable was being installed on barge winch grabline. While cable was being pulled through an 8"" sheave, EE attempted to reposition the sheave. The moving cable caught his right hand & pulled his fingers into the sheave injuring his fingers & hand. One finger was crushed with an open wound. The other fingers were also injured, but not as serious.
A rock slid off buggy, 2' X 1' X 3"" and fell on his left foot. Broke his little toe.
While dumping a load of rock & dirt, the load got hung in bed of truck, causing the trucks front wheel to come off the ground. When the load broke free, the front of the turck fell back to the ground and bounced the driver out of the seat. He did not have a seat belt on.
Employee was operating the continuous miner, when a piece of draw rock measuring 2 feet by 4 feet by 2 inches fell, strking him on his back and hips. Employee has no broken bones but is loosing time.
Employee was walking down refuse belt at prep plant. As he came to ground floor he stepped into hole caused by built up fly-ash causing right ankle to turn. He was taken to doctor and diagnosed with slight fracture to right ankle and advised not to work for three weeks.
Employee stated that he was operating a cont. miner and had backed it up out of the cut. He walked up beside the machine to look at the roof conditions, thinking he might have to trim some top. He turned around at the second row of bolts and was looking at his cable. A rock fell struck the head, slid down off the side and struck his foot.
Injured was installing belt conveyor top rollers. He lifted up on the top roller frame to position in under the top belt. He felt pain in his right chest wall. This was from strained ribs.
Installing floor grating and felt pain in his lower back.
The injured employee stepped out from under the canopy of a Fletcher RRII bolter to go to the back of the machine to get some supplies when he was struck by a piece of roof coal measuring 21"" X 15"" X 5"". The impact drove his hard hat onto his safety glasses causing a laceration to the bridge and left side of his nose that required 12 sutures to close.
Building road, employee got off of dozer to determine exact path of gas line when he tripped and fell fracturing hand.
Employee was pulling up on a 15 inch adjustable type Cressent wrench when he felt something pull in his left shoulder. He started missing work on 10/17/07.
Employee attempted to hook cable ropes onto tail of moving miner when tail struck employee. Employee sustained a simple fracture of the pelvis. This accident occurred on #2 unit at 27 crosscut, between #3 & 4 entries.
Subject was installing shear pin in feeder pick breaker. He injured left foot kicking reducer shaft (fracture & contusion).
Employee claimed he was stepping backwards and fell and as he reached back to catch himself he strained his shoulder. This employee had a previous condition with this shoulder. The difference between the date of injury and reporting date was pending a medical report from the Dr. and acceptance of the claim.
On oct 3rd, EE reported that on 9/20/07, while pulling on a cable he injured his back.
Victim was crawling and placed left knee on rock. When he put weight on knee, the knee twisted.
Replacing conveyor belt, downsizing, cutting off excess with utility knife, overexertion accidentally caused cut to left thumb. Employee had gloves on.
Working on a crusher, he overextended his right arm with elbow while using a sledge hammer.
Employee was moving rock into a bucket when he heard and felt his back pop. There was immediate pain. When walking he feels a painful twitch. Employee was sent to the doctor for an exam.
Employee was mounting equipment when his hand slipped off the handrail and he jarred his right leg as he fell.
Employee was mining and a piece of ventilation tubing fell striking employee on his head and left shoulder.
#5 Entry intersection. Hauling coal on the shuttle car from the miner to the belt. He hit a large rut or hole in the roadway. Knocked the power on shuttle car. He hurt his neck, back, hips. Could not feel his foot and right leg. They were numb.
A Sein Brute 612 mantrip carrying two miners was traveling at 38-39 xc, 592 S.Longwall Panel 3 Road (1 Room). A mechanical failure within the steering mechanism resulted in the over-turning of the mantrip. A miner was pinned under the mantrip resulting in fatal injuries. The other was ejected, sustaining injuries. The company is investigating this incident with MSHA.
Employee stated after refueling his 777C truck he was climbing the entrance ladder in order to enter the operators compartment. In the process of going up the ladder his foot became caught on the third step up and caused him to slip. He stated that he was able to hold onto to the handrail but this caused him to twist his low back. He continue to work until 9/28/07.
EE was extracting a rock that was between the right rear tires. He thought he had it out and started to pick up his tools when the inside tire blew knocking him to the ground. His hard hat went about 50'. EE got up and moved about 20 feet to the North where co-worker & Plant Manager found him.
Injured employee was standing next to the section power center. Injured employee stated that he stepped forward with his right foot placing his weight on his left foot when he heard something pop in his left knee.
Putting folding leg for crusher up in place, reached to grab bolt that holds leg in place and was hit in face by leg as it swung down.
It was raining and EE was stacking stone on a pallet. He slipped and a rock fell on him, scratching his leg. It became infected and caused an abscess on his left thigh
EMPLOYEE WAS STEAM CLEANING IN THE PROCESS ROOM WHEN HE STUMBLED AND RAN STEAM ACROSS RIGHT FOOT.
On 9/18/07 the employee got a foreign body in the right eye. He was not wearing goggles. He finished his shift & on his way home experienced pain in his right eye. He went to the ER & had particles removed from rt eye & was given Rx eye drops. A follow up appt w/optometrist revealed more particles that were removed. He was taken off work 9/19, returned to full duty 9/20.
EE was operating production FEL loading haul truck w/shotrock. A rock 2"" in diameter rolled down muckpile between L. front & rear wheel. He did not see rock and ran over the rock W/L front wheel while backing out of the muckpile W/ a loaded bucket. The resulting impact jarring motion hurt his back. Miner reported need for medical attention on 10/17/2007.
EE was trying to lift a large stone and strained his back.
As the EE was getting dressed to report to work, when he went to pull on his leather boot he felt a sharp pain in his lower back.
Unknown - employee said that it may have been from the 2 previous vehicle accidents he had been in.
Dozer operator failed to wear seatbelt while pushing stripping material. Operator was traveling to lower area when he missed ramp. Dozer went off a 3' embankment and operator was ejected from seat and resulted in a cut on his forehead which required stitches.
Person operating continuous miner on south section, #7 entry taking B run. Operator was setting 6 rows of bolts outby last row. Rock fell from between rib and 1st row of bolts and hit operator in shoulder, wrist and leg.
Injured was walking thru the crosscut toward the slope belt. Injured struck his head against an ""H"" beam in the mine roof. This resulted in a cervical and lumbar strain.
Employee stepped off skid-steer and twisted his right ankle.
Employee was lifting the rub rail on the miner. He pulled his back. He finished the shift. As he left the lamp house, he told his foreman he hurt his back. That is all the information he gave or has given.
Injured was helping to pull a pump cable which he had thrown over his shoulder. As he was pulling the cable around a timber he felt a pain in his shoulder.
EMPLOYEE WAS GETTING OFF DOZER. HE STEPPED DOWN TO THE TRACK WITH LEFT LEG, HIS RIGHT FOOT CAUGHT ON THE EDGE OF STEP & HE SAT DOWN ON RIGHT LEG. HE DIDN'T FALL, HE OVER-EXTENDED RIGHT KNEE.
The employee was installing the last bolt in the face, he placed the glue and bolt into the hole and hit the fast feed on the droll pot. This caused the canopy to drop 3 to 5 inches and strike the ee in the head. The canopy had been raised by the drill pot pressure on the floor and was not defective.
Employee was sitting down welding on a mill part (spider) when slag from the welding rod fell inside his laceup boot burning the inside of his left ankle and bottom of left foot.
The 988 coal loader was chopping coal to load and the 992 spread was working same pit. As 988 was backing up chopping coal, the bumper of the Loader hit the bumper on rock truck.
While watering roads, operator ascended hill towards the rock truck dump. Mid-way the engine of the water truck stalled, causing the water truck to roll backwards down the previously watered roadway. Operator lost control and the rear driver's side tires encountered the safety berm causing the truck to roll over onto it's side.
Employee slipped on chunk of coal while examining and felt pain in her back.
Mechanics were attempting to remove a seized travel brake assembly on a continuous surface miner. When the brake was removed, the track moved suddenly, striking the victim on the left arm. The resulting wound required three staple type sutures.(ee was unable to report for duty as a result of the injury beginning 10/6/07)
Trimming corner from block, Continuous Miner head caught RB cable, cable fell onto and pulled operator against CM.
Employee was carrying electrical cords and felt a pain in his back.
While operating a 5 ton chainfall, employee felt a nip in his stomach.
Lifted a 5 Gal. bucket and complained with his back.
Employee was carrying 25 lb rock when he slipped and rock fell on hand injuring his right middle finger.
While welding & using torches on crusher, slipped & fell on rear end.
Miner was down wind of electrical cleaner that was used on a power center. Note: It was not concluded until 10/1/07 that days lost were workers comp related.
EE fell while carring a bucket of plaster. which had the lid off of it. The brown liquid on top of the plaster splashed up into his lt eye.
EE was removing the salt spreader from a truck. Took a few steps back & sat it on the ground and had a pain his lower right back.
Injured was going through mandoor to check pump when mandoor shut on right leg.
Loading top belt rollers into the bed of a coal hauler. Starting missing work on 10/01/07
EE was putting hose for treatment of water in ditch, when the hose momentarily got caught on pole that holds the hose when not in use. When the end of the hose came loose it shot water into his right eye. Took him to hospital, was told to take 4 days off.
EE was grinding on a beam when a sliver of metal flew into his eye. He had safety glasses on but the sliver got into his eye anyway.
Employee was using a pry bar to lift and align a transition chute for installation from screw conveyor to an inlet for the silo when the pry bar dislodged while applying pressure to align the unit. Employee slipped and fell backwards onto a sharp metal object.
He was changing a cable on an automatic recoil device on a crane tagline. When the dogclutch of the recoil device was released, we believe the cable caught employee's right leg and threw him into the air. He received contusions and abrasions to his right ankle, knee and shoulder.
EE was checking a tie rod on a utility truck and got some dirt on his eyebrow. He removed his safety glasses to wipe his eyebrow & knocked dirt into his eye. He worked the 16th and 17th of Sept before going to a doctor on the 18th. The doctor referred him to a specialist on 9/22/07 and he was given time off for a scratched eye.
He was tramming the Shearer to the tailgate when a bump occurred. A chunk of coal struck the left side of his Airstream helmet face shield, striking his cheek. He did not go to the doctor and continued to work. On 9/21/07 he was diagnosed with a fractured bone below his left eye, surgery is required.
Employee was closing the door on a crusher tub for removal and lost his footing on the step plate, and fell 3.5 ft. onto the walkway below. There was loose gravel all around due to a crusher breakdown.
Injured employee was dismounting the rear of a grader after checking the engine oil. As he placed his right foot on the ground, his ankle gave out causing him to fall and fracturing the right ankle. The ground surface was smooth where he stepped. He may have stepped down on the side of his foot.
Making a belt move, pulling the conveyor belt through the tail piece, the clavis that attaches the wire rope to the belt puller became fouled up on the crossbrace of the v-scraper. He placed his foot on the conveyor belt to dislodge the stuck clevis. The clevis released and the belt puller and conveyor belt pulled through and caught his right foot between the cross-brace and belt.
Employee's knee locked when trying to step out of ditch on the tailing dam after cleaning out the end of a 6"" drainage line. He was taken to ER and was taken off work pending further diagnosis. Ongoing.
Roof fall in #3 heading of 4 left in the 1 E Butt. Fall was 30' wide, 50' long, and 12-15' high. Miner operator was turning crosscut 3-4 when a roof fall occurred. Operator dove under boom for protection, however some rock fell on his right leg.
EE was descending a slope when EE slipped and fell at same level. EE initially thought was ok, but went to stand up and felt a pain in right knee and leg. After diagnostic treatment EE has quad tendon tear in right knee.
Employee was installing roof bolts and while bending a roof bolt he felt a pain in his back. Did not become reportable until 10-17-07.
DID NOT START LOSING TIME UNTIL 11/1/07. Slipped on a piece of belt and felt pain in left knee. Surgery done 11/1/07. Worked until that time.
Man was kneeling down to begin working on shear at LW face. A rock fell out of the roof & struck him in his right leg.
The injured employee stepped up to the miner to check the pressures on the mobile roof supports when an undetectable rib and brow fell striking the employee on his back and his legs riding him to the mine floor. The rock brow measured 7.5' long x 14"" wide x 0-16"" thick.
EE was disconnected a jumper hose that had caustic accumulation in it. When EE disconnected the hose, caustic got onto EE's face and upper torso. EE was taken off of work for two days by his treating physician as a precautionary measure.
At the time of the incident EE was exiting a service truck. As he stepped down on to the rough ground his right foot buckled under his weight.
Employee's right ankle got wrapped up in a piece of 2"" waterline that was hooked to the section pumping station. As it was being pulled up to the face by a scoop.
Employee was walking from H-Main Seal area where the walkway was very muddy. Employee slipped, twisting left knee.
#2 section, pulling on feeder cable, employee was dragging feeder cable and he twisted his back. He felt something pull in his back.
Employee was riding on mantrip when his CSE SR-100 he was wearing on his belt flipped over off the side of the mantrip and was caught between mantrip and portal canopy. His right arm struck the canopy cutting his arm. Miner proficiency a factor, mining equiment could be a factor, procedures and policies not a factor.
Employee was moving the Odex hammer when he stumbled and fell with the hammer landing on his right arm. Diagnosed with right ulnar nerve contusion. Prescribed medication and ice for injuries. Placed on light duty.
Member was removing the pin from a hi-wall miner beam, when he turned to throw the pin he slipped and fell, landing on is hip and tailbone.
Employee was climbing into the operator's compartment of the loader in #7 to #8 crosscut, 3 North Right Section and tripped on loader cable twisting his left ankle.
EE was sweeping out the drill cab. When finished, he reached out with his right hand to grab the door handle, while bracing himself with his left hand against the door frame. As he pulled the door shut, his thumb was pinched between the door and the frame, fracturing his thumb.
Moving toilet tank & hurt lower back.
Miner was operating haul truck approaching a curve when another operator of a haul truck approaching curve from the opposite direction failed to maintain control of his truck and skidded sideways in front of vehicle striking the cab with left rear of truck bed entrapping original miner and causing a broken leg.
Miner was walking; foot slipped twisting ankle. Note: began missing work 9/19/2007.
Bolting intersection in #3 heading spad #1305. Employee was installing a combination bolt with his right hand on the pot and the corner rolled off on him.
Using shovel to clean chute slide, more dirt than the shovel could hold came down and hit shovel. Shovel and hand on handle hit steel on crusher.

Employee was helping to replace a cylinder jack on the left side bolter on the miner. Mast assembly fell striking employee's right leg.
Employee was dismounting drill. When he put his body weight on ladder the ladder broke causing employee to fall and land on back.
Employee was moving a stone to pick up a piece of board. He felt pain in his lower back. Diagnosis: Back strain - restricted duty assigned.
EE felt a pop in her thumb while turning off water valve. EE worked until 10/10/07, when she went off for medical treatment, thus making the injury reportable.
The victim had just walked over to the power center and was talking to co-worker when a piece of rock 13 x 30"" x 9"" fell striking the victim in the head and shoulders.
Employee was shoveling belt head and pulled lower back.
The individual was walking the 913 refuse belt looking for bad rollers. He observed a build-up of mud on a roller. He tried to remove the debris with a wrench. His arm was caught by a roller and pulled in.
Following a fuel delivery to customer tanks, EE was returning his 3"" x 20' hose to the storage tube when he slipped on the large stone drive surface. In a attempt to break his fall, he stuck out his arm and felt a ""pop"" in his right shoulder. Reported to Progress Coal.
Employee was walking across section and stepped in a hole causing him to twist his knee. His first day off was 10-8-07.
EE was on the section mantrip in the #4 entry adjacent to the #3 belt traveling toward the section, when the rubber tire mantrip hit a hole and caused him to absorb the shock, resulting in an injury to his lower back and right knee. EE had an injury on his right knee on 7/28/07 and an injury to his lower back on 4/3/07.
The employee was using a water blaster to dislodge material from the Preheater. He later complained of pain in his right arm and shoulder.

The employee was being task trained on the drill when he went to get in and shut the door. The sliding door jamed halfway shut, and he started to pull on it. The door broke free and he caught his hand in it.
#3 entry, 6 breaks outby face. EE was getting out of forklift, when his foot slipped. He fell back and hit his head on a rock on the ground. His foot was hung on break tender.
Employee was in the surface shop installing a transmission in a underground mantrip. The transmission became hung and wouldn't go in any farther so he was pulling at it trying to get it to free up. All of a sudden it came loose and caused the employee to fall backwards, twisting his knee and striking his elbow on a metal cover. He started missing 9/19/2007.
While moving boxes and furniture from second floor office to first floor office employee began feeling pain in neck, shoulder and back.
Employee was unloading a box of glue and twisted his lower back area.
The injured EE was working in the heavy machine (D-6) and when he was got off the machine he sat on the floor and had pain in his lower back.
Employee was climbing down off of the continuous miner in #1 Entry, 3 North Left Section. His left foot caught on a water hose causing him to fall to the mine floor jamming his left wrist into the rib, resulting in fractures to the left wrist.
Hose blew off the steamer @ heater coil. Employee was burned/scalded.
Employee was working on 4400 TK bed. He was carrying a piece of steel, slipped on wet bed (from dew) and tried to catch himself from falling with his left hand. It bent his little finger backwards. Doctor said it tore tendons and for him to take two days off.
While working on a 6 VDC battery, tool contacted both positive and negative posts causing one cell to arc and spray battery acid into eye.
On 9-12-07 the employee was walking in entry and stepped in mud with his left foot. As he attempted to take his next step his left foot remained stuck causing him to twist his left knee. He continued working until 10/31/07 at which time he started missing work due to pain in left knee.
Moving wheel unit out of way and pulled lower back.
Was walking around a piece of equipment and foot slipped. He fell & hit knee & shin on edge of concrete pad. Initial treatment by Dr. was first aid, Ace wrap & no prescription meds. Accident became reportable on 9-20-07 when employee returned to doctor & he determined hematoma needed to be removed.
On 09-11-2007 Miner was climbing back in to haul truck after fueling. While climbing and pulling self up onto first rung of ladder, miner felt a pulling or tearing sensation in the groin crease of his left leg. This resulted in an Acute left groin strain. Miner first reported incident on 09-12-2007
EE stated he was bending a roof bolt, when he felt a sharp pain in his lower back.
Employee was exiting vehicle and stepped on uneven surface causing him to lose his balance, reached for door to vehicle catching himself, causing strain to lower back.
Employee was leaving the premises at the end of his shift. He pulled on the chain used to lock the front yard gate. He heard and felt a ""pop"" in right arm. He pulled his tendon off of his right bicep. Please note that the date of this injury was also his last day of employment. He had previously given us his notice that he would be seeking employment elsewhere.
Employee was adjusting brakes on water truck - adjusting in wrong direction. Truck rolled - axle housing hit his left shoulder.
CLIMBING OUT OF THE LOADER, FOOT SLIPPED ON A STEP AND HIT HEAD ON SIDE ENTRY STEPS, THEN FELL DOWN TO THE GROUND.
While riding to the section, Employee was struck by a piece of draw rock. He was transported to a local hospital where he was examined, released and returned to work but was scheduled to see a specialist. On Tues. 9/17/2007 he was diagnosed with a broken collar bone and is scheduled to be off work for four to six weeks.
The maintenance employee was working on Screen 2 (after it was raining) and slipped on surface causing a strain in the employee's back.
Injured employee was operating battery powered scoop when the front tire struck a pothole in the mine floor causing his head to strike canopy of machine. He was seen in local ER & issued full release to return to work. Later visited family dr for follow up where MRI was ordered. After reviewing the results of the MRI the family dr. ordered regimen of physical therapy.
Employee was cleaning floor of 777 Cat truck and developed pain in his back. EE went to Dr. on 9-12-07 and had an MRI performed on 9-21-07. MRI results were read on 9-27-07. Employee is off work.
Employee was struck on the back with a piece of slate.

Employee was lifting a 6' step ladder out of the mechanic's truck and felt pain and burning in lower back. Employee began losing time 3-25-08.
After working on fender lights of equipment, employee stepped from fender back onto deck, hitting and slightly cutting his face above the right eye. He then fell to the ground.
Turning corner to haul up #2 when rib and corner fell out and into the deck of shuttle car, injuring operator's right knee and lower leg.
The employee was shoveling the #3 head drive when his shovel became hung in the belt splice and he allegedly twisted his back.
EE was moving a slab of sandstone into position to be split, when he cut his left hand on a sharp edge of the stone. EE was given a tetanus injection, as well as several stitches to close the laceration.
EE was moving a portable work platform when one of the platform wheels got stuck on an adjacent structure. When EE tried to dislodge the platform, he felt a pain in his left groin area. On 09/25/2007, EE was diagnosed with a hernia and removed from work.
The employee was taking structure out of the belt when a rock fell out from between the bolts and allegdly struck him on his left hand.
Employee was cleaning the coal mill building and was filling 5 gallon buckets with material carrying the buckets down the steps and dumping them into a trash hopper, Injury to right bicep. Pain is now going into the right shoulder. The accident was not reported to the co. until sept 12. pain increase sept 19-send to dr.
Employee was putting a bolt in a scalper. He was pushing on the scalper and felt a pain in his lower leg/heel area. He was evaluated by a physician and was taken off of work.
Employee was pulling rockdust hose and tripped on a 4"" Beam uder rail and fell hurting her back
A 14' camper trailer used as a guard shack. Employee was using a propane cook stove to prepare a meal and apparently fell asleep. The trailer caught fire. He escaped trailer but suffered severe burns to hands, arms and neck and minor burns to his face.
Employee was driving a haulage truck and hit a bump, which caused his back to wrench slightly.
Employee was transported to another location by a different driver. Employee had climbed down the ladder and placed her backpack on the ground. Employee got back on the ladder to get her lunchbox when the truck began to move. Employee fell to the ground and crawled out of the way of the haul truck. Employee was re-evaluated and taken off work on 9/20/07-Mentally Unstable.
EE fell while cleaning bottom of thickner at the prep plant. He strained his left shoulder in the process. EE started missing work on 9/10/07 due to left shoulder pain.
Employee was removing a hydrualic hose from a cylinder on a 330LC excavator, when the clamp came loose. Hydraulic oil, under pressure, hit him in the right hand, causing swelling and pain in his hand. No oil was found under the skin. Employee went to the hospital and his legs became swollen for an unknown reason, causing him to lose time.
Picked up blocks and positioned them on the ground and lowered pump onto blocks from boom.
Miner was attempting to hang tire on rib when his left shoulder gave out. The night before the miner reports that he was changing a drill steel for his partner when a loud ""POP"" came from his upper shoulder but didn't think much of it.
While unloading a 30 foot length of pipe, the end of the pipe toward the front of the vehicle slipped. The rear of the pipe ""whipped"" around, striking employee in the jaw and face.
A top sheet failed to cut off completely. Employee opened the gate to the restricted area, overrode the electrical safety switch by blocking it off and proceeded to dislodge the top sheet while the machine was operating. The top sheet dispenser returned trapping his arm and shoulder against the metal structure.
EE was running a skid loader. He stopped it with the bucket up to scratch his leg. He put his foot outside of the cab of the loader. He moved his right foot that was controling the bucket lever and it fell on his left foot.
Employee was grinding a light bracket for on a scoop when he felt something enter his right eye. Employee was wearing safety glasses when this occurred.
Employee was in the process of cleaning up coal with the miner when a piece of rock fell from the roof and struck him in the head, shoulder and back.
The injured was checking the oil level on an air compressor which had stopped. When he went to remove the oil cap to check the oil level, hot oil blew out of the compressor striking the individual on the face and arm.
EE walked between two pallets while on his break. A rock fell off one pallet pinning him to the second pallet. He does not remember leaning against the rock that fell.
Employee was using the grinder on a metal collar and a shard of metal came up and hit his right eye.
Miner was kneeling down looking at the work area when he felt a pain in his back.
Sprained right ankle. Caused by employee bouncing on foot to give momentum to climb ladder into water truck cab. Ladder had been previously modified at this employee's request to make access easier.
Two employees were using an excavator to lift a dump truck bed from the truck body. One employee was in the bed, hooking up the lifting chains when the operator accidently moved the bucket striking the first employee.
At end of shift, employee reported pain to his right shoulder. During shift bending 6' resin bolts. This employee suffers from chronic arthritis. His injury was denied by mine & Worker's Comp. carrier. Submitting form in protest to terminate citation.
Injured EE and another EE were in a manlift going to the top of the bagging elevator. The basket shifted when they turned and EE's hand was caught between the basket handrail and a walkway handrail. About 3/4 of EE's finger was peeled back.
EE stated, while his truck was being loaded, a rock rolled & struck the inside of the bed of his truck & caused a severe jolt to his neck & then when he went to dump the load, felt pain in neck. When he returned to the loader, he contacted his foreman & requested an injury report be filled out; he completed his shift & drove himself home.
Employee stretched to hold a belt clip and felt a pop in left side of neck.
Employee was driving a 777 rock truck when he drove through the berm and turned over. Employee complained with shoulder pain.
Employee stated he was hanging the continuous miner cable. felt pain in his back started missing work on 9/8/07
