The fall occured in the #3 entry approximately 1600 feet from the surface on 1-A belt roadway. Fall is about 40' x 20' x 2 to 4 feet thick in a remote neutral area and will be timbered off.

A roof fall occured in the #9 seam in the 3 East Panel in the #4 entry inby the the intersection of crosscut #27 and extending through the intersection of crosscut #28. The fall measured approximately 80'x18'x7'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.
No damage. Fell intersection a thick rider was present. approx. 24"" thick about 6' high.
An unplanned roof fall occurred in the #130 mine on the #7 belt, #7 brk in the #6 entry. The fall measured approx 60' long X 20' wide X 10-12' high. No injuries were reported. The area was developed 10/26/06.
A roof fall happened on the 5W due to weak shale.
An unintentional roof fall occurred at the #3 Bleeder Entry 275N (behind the longwall set up face). The fall was approx 40' long, approx 18' wide and an unknown thickness. The fall did not affect ventilation or passage of EE's. The fall will not be cleaned up.
A rock fall occurred on the head of the continuous miner during retreat mining.
Elevator control cable was cut while repairs to the compensating chains were performed.
Roof Fall #4 Entry. Fall was about 30' X 30' X 6' thick. Fell up to the coal rider.
Rock fall occurred in the 3rd northeast panel at spad 3+50. Roof is of shale type. Fall size is app. 20' x 65' x 6'high and app. 6,000 ft from active working faces. Fall occured in our # 5 entry of this panel.
The hydraulic take up malfunetioned causing the drive pully to slip and create enough heat to ignite the belting on start up. No injuries were involved with the event. The fire was completely extiguished in 41 minutes from discovery.
A non-injury roof fall occured in the # 2 entry of 12 west at crosscut 25. The fall measured 8' to 9' high, 35 feet long and 18 to 20 feet wide.
A roof fall occured on the 2R1L/4W XC9 entry #5 on the old 4B roadway.
A loaded blast hole was ignited when struck by lightning.
A roof fall was found during the weekly examination in the old works of the short balck mains. Located in break 10 in the #3 entry. The fall is approx. 7ft thick by 40ft wide by 60ft long.

Old North. 8 belt #9 entry, MMU 004, 4-way intersection spad 17495 20ft wide x 20ft long x 7ft high. Support: 6ft 3/4 inch resin bolts, 12ft cable bolts, pans, brow straps, screen. Fall danagered off, additional supports installed around fall.
An unplanned roof fall occurred in the #2 gas mine at brk #16, #2 section at spad #678 that measured 20' long x 19' wide x 10' high. No injuries were reported. The area was developed on 4/17/08.
A roof fall occurred in the #10 entry on the old #3 section panel 2 xcut inby spad #44897. The fall was 20' by 2' and 7' to 8' high. The area was bolted with 5' torque tension bolts. The fall won't be cleaned up. The area will be timbered and dangered off.
A roof fall occurred on the 4W at XC #153 entry #2-1.
A two-fold electrical problem occurred with the Koepe hoist. The lower landing stopswitch had a malfunction, and wouldn't allow brakes to release on manual. Also, the re-synch motor that controls the lily controller was not functioning properly. Mine was evacuated and hoist back in service about 1:45pm. No injuries.
A roof fall above the anchorage zone which impedes access to the face occurred on the c-1 longwall section @ approx. 3:10 am on the stageloader. The fall is approx. 33 feet long 16 feet wide and 8 feet high. All agencies were notified according to law. Mining and roof support activities continue according to law.
While traveling airways in the old 5Lt. Section with an MSHA inspector a roof fall was discovered at spad #8016 in #3 entry. The fall measured 20'L X 20'W X 4' high. The area was supported with 4' resin bolts. The area has been dangered off and timbers have been set. The area was mined approx. 4/07.
A cribbed off area between and old travelway and belt entry that will again be used in future had a slip fall in and pulled 6 bolts 10' X 15' X 10'. This was between #5 & #6 entry in 2nd west.
#3 hoist kicked out on loading pocket fault then on east motor over temp. Vibration trip south skip spear point - reset vib trip - east motor over temp at 17:10 hours. Still had two operable hoists.
A roof fall occured in a 4 way intersection # 10 Hdg 52 x-cut 3 mains. 18 x 18 x 12' high. 4 x-cuts outby loading point. Intersection was bolted with 6' super twist + 4 @ 12' post tensioned cable bolts. No ventilation, escapeway or travelways were affected.
Employee was in the process of exposing a void in a stockpile above an underground feeder when his dozer slipped partially into the void.
While traveling air ways discovered a roof fall 1 break to the right of spad #268. It was measured about 20' wide, 70' long and 7' thick. Check test holes around it and checked OK. Timbered and dangered off. Looks as if 2 hill seams caused fall. Will keep a closer look at top to try to keep this from happening.
Loss power to primary hoist due to offsite power supplier transmission line shorted that left the mine with out power
The Cedar Creek Elevator was out of service from 10:00 pm to 11:00 pm due to a frayed electrical wire.
A rock fall was found in old #1 units return between spad 4 + 20 and 7 + 82. The fall was 4-6 feet high and 19-20 feet wide. This area is 3 xcut from return travelway. 1st East Panel off 1st Panel South - (MSHA on site)
A roof fall occurred on the 5WB at XC#75 entry 3-4.

While performing a preshift examination of the recently constructed seals on 3 right off 3 east mains, the examiner discovered a thermal event within the #6 seals. Upon close examination of the seal, it was ascertained that a fire was within the seal. Efforts to extinguish the fire lasted approx. 28 minutes.
A rock fall was discovered in the 1st south east mains in a mined out area during a weekly examination. The fall is one crosscut inby spad 556 and 564 across the old #3 and 4 entries. The fall is apx. 140 feet long x 20 feet wide x 7 feet high. No persons, equipment nor ventilation were affected. The area has been dangered off.
Rock fall occurred in the 3rd NE panel at spad 37+86. Roof is of shale type. Fall size is approx. 164'20'x10'High and approx 3700 from active working. Fall occurred in the #5 entry of this panel.
A roof fall was discovered in an abandoned panel. 1st East off 1st south off main street. The fall was located in #2 entry at spad 24x90 and was 20' long x 80' wide into crosscuts x 6' to 8' high.
An impoundment failed allowing water to flow eventually into a nearby creek. There were no injuries or property damage.The water flowed into a low lying area first and then into the main settling ponds and then into a creek. There had been an great deal of rain in the past two weeks.
In the 6th left Butt in rooms #17 to 18 200' inby #2823 spad. There was a roof fall between #17 and #18 room. Fall was 6' to 10' high, 18' wide and 30' long. Area was bolted with 6' fully grouted rods. This was in a transitioned roof area. There is some slick and slide rock in fall.
An unplanned roof fall occurred in the #2 gas mine at spad #734 in the #7 entry that measured 28' long x 20' wide x 14' high. No injuries were reported. The area was developed on 5/14/08.
An unintentional roof fall was found during the weekly examination of 002 Bleeder panel. The area has been cribbed off to prevent access to the area.
An unplanned roof fall occurred in the #2 gas mine at spad #707 in the belt entry that measured 28' long x 20' wide x 14' high. No injuries were reported. The area was developed on 4/23/08.
Rock fall occurred in the 3rd northeast panel from spad # 30+66 to 33+06. Roof is shale top. Fall is 19' x 270' x 6' plus. 2800 ft from active working faces. Fall in #7 entry of this panel.
A roof fall occurred in the #1 entry return 2 breaks outby survey station # 935. The fall was aproximately 10' thick x 18' wide x 70' long. While investigating the roof fall, a second fall was discovered 60' inby survey station # 911. This fall was approximately 8' thick x 18' wide x 90' long. Both roof falls were aproximately 2000' from the working section.
#3 hoist kicked on east motor overtemp. Maint coord and electrician came out of the mine and turned on blower fans and opened the doors to cool the area down. The hoist was reset and back in service @ 4:45 pm. There were still tow operable hoists.
Rock Fall occurred in the 4th Southeast submains (+3406) Spad #6 - #7 Entry and adjoining x-cut. Fall 6 ft plus in height by 19 ft long by 130 ft wide. This area is approx. 6,500 ft from active working faces. Type rock - Shale.
A ignition occurred on the 6-mains section, approx. 65 feet in 24 block, 1 entry. Shuttle car was loaded from this cut and ee was trimming the top before the second shuttle car arrived. EE seen an orange ball that started on the curtain side of the miner head. He put his head down and when he looked back up, it was gone. Bits striking sandstone appeared to be the was cause
Fall was discovered in the roadway on allor mains at break #61. Fall is approx 25'long, 18'wide and 3 1/2 to 4' high.
An unintentional roof fall has occurred between the #3 & #2 entries at 27+60 on the 2 mw in the return. The fall was discovered on the weekly exam. The fall was approximately 20' long and 7.5' high and approximately 16.5' wide. The intersection #3 had been cribbed prior to fall, from the intersection of #3 to the #4 entry.
A face ignition occurred on the 003 mmu at approx 12:10 am. No heat of concussion was felt by witnesses. It self extinguished in seconds. No injuries to persons or property. Sandstone was present.
There was a roof fall discovered on North tailgate one break inby spad #10147 between breaks 81 and 82. It measures approximately 25' L x 20' W x 8-10' H.
An unintentional roof fall occurred at the Galatia Mine New Era Portal at the #3 Entry West 3 West Headgate, Crosscut 16, 2172' West. The fall was approximately 35 feet long, approximately 17 - 19 feet wide and approximately 3 - 5 feet thick. The fall will be cleaned up. Fall # 25G
During the normal weekly exams of the production hoist, it was determined that the North skip rope met the requirements of 57.19024 for retirement/replacement. The mine was evacuated (39 persons). No injuries. No other problems.
#3 hoist kicked on east motor overtemp. The maint coordinator and electrician came up and turned on blower fans to cool the motor and reset the hoist. Hoist back in service @ 4:20 pm. Still had two operable hoists.
A fall of roof occurred at the intersection, in # 4 entry, 21 cross cut in the South East. The fall was approximately 35 feet in length x 16 feet in width x 8 feet thick. This area was develpoed in April 2004 and had been supported with 6 foot fully grouted roof bolts.
A roof fall occurred in the track entry at x-cut 64 on the 2 south track. The area was bolted with 5 foot torque tension and spot bolted with 10 foot rope bolts. The fall will be cleaned up and rebolted and additional supports where needed. The fall was 20' by 20' wide and 6' to 8' high.
A continuation of an existing reported roof fall was found by examiner. The fall is located in a set of abandoned rooms at 1+80 from #3 to #6 room, #6 to #7 room & from #7 to #8 room off the return side at 29+60 to 30=80 of zone P1 of zone sub 1. The new fall is 265'L x 18.5'W x 8'. The area was originally supported with 5' resin bolts. No injuries, air flow was not affected.
A roof fall was discovered in the 2 North 3 Mains, XC 36, # 4 Entry. The fall measured approximately 60 feet long, 20 feet wide and 12 feet thick. The area is supported with 5 foot fully grouted resin bolts and 12 foot cable bolts.
A broken spring was found during the preshift hoist inspection. No persons were underground. While the hoist still was capable of functioning, it was downed for repairs. The hoist was returned to service after the spring was repaired and proper function tests were successfully performed.
An unplanned roof fall occurred in the coalburg extension mine in the #3 entry. #3 brk at spad #242. No injuries were reported. The fall measured 50' long x 19' wide x 6' high. The area was developed on 5/2/08.
Weathered stack shale, mixed into stack sand stone., seperated in L line of x cuts, allowing roof to fall. Fall was found on a weeklt exam.
At 11:00PM distpatcher reported that at the CO sensor located at the 8HLG#2 Tailpiece. EE was sent to the area to check out the high alarm. When he came around #66 Break he saw flames. There was a fire extinguisher at a pump. He put the flames out. The tailroller bearing failed, it was changed out.
The mine encountered a fall of roof on the 1st MS, entry #4, S.S. 21+10. The fall measured 50'L X 18.5'W X 12' thick. The area was developed on 12/31/07. The fall did not affect ventilation or passage of men, no personal injuries or equipment damage. The area was supported with 8' point anchor bolts. This fall will not be cleaned up.
A roof fall occurred on the 3 Main section #5 Hdg 46 x intersection 10 x cuts outby the loading point. The area was 20' long 18' wide 9' high. It was bolted with 6' fully grouted super twist bolts and 4 - 12' post tension cable bolts. No ventilation, escapeway or travelways were affected.
An unplanned roof fall occured on the number 1 section right return # 7 entry, approximately 900' outby the working section, near spad 495. No accidents reported. Area developed 03/01/2008. The fall measured 40' long X 18' Wide X 10'-16' thick.
Employee found a reportable roof fall in E-1 Headgate, entry 3, crosscut 27. The fall was approximately 22 feet wide, 25 feet long, and 6 feet high.
An unplanned roof fall occurred in the # 2 Gas mine, # 1 Entry at spad # 683. The fall measured 20' long x 20' wide x 15' high. No injuries were reported.
An unplanned roof fall occured just inby spad #1845 in the #5 entry the fall measured 150' long by 20' wide by 5' high no equipment or persons were involved ventilation was not affected a clean up plan was submitted and the fall has been rehabilitated.
A fall of roof occurred at the intersection, in # 3 entry, 2+70 in the 1st left panel. The fall was approximately 60 feet in length x 30 feet in width x 8 feet thick. This area was developed in October 2007 and had been supported with 5 foot fully grouted roof bolts.
3rd shift hoist operator had the hoist stop running- an electrician was called out, MSHA was notified and the foreman began the process of evacuating the mine. The electrician arrived 3:00AM and determined immediately that the problem was not a failed drive, but an issue with the PLC. The PLC needed the program rebooted. Evacuation was cancelled as the hoist was functioning.
Face ignition lasting 2-3 seconds no injuries.
At 3:25PM it appeared that the circuit board for the Avtron hoist failed. The procedure to replace circuit board takes over 30 minutes. MSHA was notified at 3:30PM that our hoist was down and the miners would evacuate using the emergency hoist at the #3 shaft.
Underwind fault, recent hoist automation ""bugs"" being discovered and corrected.
On 5/13/2008 there was a frictional/methane face ignition in the 10B Corridor, 082MMU mining section. There were no injuries to persons or equipment damaged as a result of this accident.
While making a pre-shift examination of a old set of rooms employee found a roof fall around 4:30 am at intersection of #12 room and x cut 12-11 and part of x cut 12-13. Set posts and cribs and dangered area off.
Unplanned roof fall in room #2-3 of the 1st north butt in return air course. The fall was 20' wide x 80' long and 42"" high. The roof was bolted with 42"" fully grouted resin bolts.
During a weekly examination of the airways a roof fall was observed inby spad #407 (No.1 entry return) approximately 4,800 feet outby the face. The fall measured 35' long, 18' wide and 3'high. The fall is dangered-off and all sides timbered. The fall will not be cleaned up. No injuries and no equipment involved. Did not affect travel or ventilation.
Roof fall at the 3L4W at XC#27 in entry 5-4. Fall occured due to weak shale top.
A 3' x 3' x 8' deep void occurred when floor fell into the lower seam at no. 98 crosscut in no. 3 entry. The air is ingassing with 0.0% Methane and 20.7% Oxygen readings.
Roof fall was discovered at 8:30 A.M. in #9 entry at #5 belt at spad #17682. Fall was 60 ft by 20 ft by 10 ft. There was no personal injury, in the return airway, no equip involved, & ventilation or person passage not affected. Area was breakered off. Area will have additional support added, and rehabilitated. Both MSHA & OMSL investigated.
A fuel truck pulled up to the fuel tanks. when the driver got out he noticed smoke & fire coming out from behind cab. Called for help. Fire Dept arrived about 15 minutes later & extinguished the fire. No one was injured. There was only about 150 gal of fuel in the tank at the time of the fire.
The mine encountered a fall of roof on the 3rd MNW, entry #3, S.S. 48+50. The fall measured 20'L X 19'W X 8' thick. The area was developed on 10/1/2006. The fall did not affect ventilation or passage of men, no personal injuries or equipment damage. The area was supported with 8' point anchor bolts. This fall will be cleaned up.
Unintentional roof fall discovered this date. Fall location: 2 Lt section, 27 break, 7 entry, inby stations 4315 and 4342. Area mined on or about 3-21-06. Fall measured approx. 36'L X 18'W X 8'H. Previously supported with 6' resin grout roof bolts and 10' cable bolts. Overburden approximately 260'.
A roof fall occurred on the south section MMU 002-0 #4 Hdg 15x southH. 4 way intersection fell in 18' x 18' x 7' high. 6' fully grouted resin bolts were being used. No one injured or trapped. No ventilation or travel was affected. No escapeway was blocked.
A contractor snow plow operator, struck a natural gas line, located at the northwest corner of the electrical maintenance storage building. A gas line inside the building, which was damaged by the impact, released natural gas which was ignited by building heaters, causing an ignition, which was followed by a small fire.
Had fall in #7 entry brk 10 1 brk inby spad 43. Fall was approx 11 brks outby section 001 in intersection & hdg, measuring 25' x 20' w x 6' high. Area of laminated roof conditions. Areas have been heavily cribbed.
A roof fall occurred in the track entry of 1 left section. The fall measured 20'x50'x10'. No injuries occurred as a result of this fall. This report is being filed due to a Part 50 audit.

A roof fall was found by an examiner during an idle day fire check. The fall was located at 40+60 #2 Entry of Zone SUB1 on MMU-001. The area was developed 11/29/07. The fall was 40'L X 18'W X 8'H. The area was originally suppored with 6' fully grouted resin bolts. There were no injuries involved, air flow was not disturbed and the passage of men was not affected.
A roof fall occurred in the number 1 entry of the 1 Left panel. Fall measured about 10 ft high,20 ft wide and 42 ft long. No injuries resulted from this fall
This is a report a continuation of a rock fall reported at spad 511 on 5-6-08. The rock fall has extended toward the south west mains to spad 601. The fall has not affected persons, equipment nor ventilation.
The hoist was descending the brakecar when the hoist shut down. A 800 amp fuse blew, after replacing the fuses the breaker would not re-set (kept knocking). When the fuse blew it arced across the back of the Siemens electronic computer controls. A new control box had to be replaced and programmed. Cause of this was the arc shield was found to be missing.
North 7, 38 to 39 block, 6 entry MMU 003-0, spad #14699. 20 ft long x 18 ft wide x 7 ft high. Support: 6 ft 3/4 inch resin bolts, screens, pans, brow straps. Fall dangered off, additional roof support installed around fall. Clean-up plan developed and posted at fall site.
An unplanned roof fall occurred in the #2 Gas mine, # 2 section, # 6 entry at spad # 739. The fall measured 42' long x 19' wide x 13' high. No injuries were reported.

During a routine inspection of the escape shaft, a damaged guide rope was identified on the mancage escape shaft. Miners were evacuated from underground. The escape shaft hoist was back in service at 4:30 pm.

An unintentional roof fall occurred in the 3C return entry #1 at 36+45. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 22 foot x 19 foot x 6 foot high. The fall will not be cleaned up. The roof had been supported with 4 foot roof bolts.
An unplanned roof fall occurred in the # 2 gas mine, #4 entry, spad # 684. The fall measured approximately 28' long x 22' wide x 13' high. No injuries were reported.
A roof fall was discovered by the weakly examiners in the worked out area of old unit 4 on the 4W.

During shift change 1st to 3rd shift, the Koepe hoist stopped operating,was electrical problem & mechanical problem with lily controls.After re-adjustment of over-travel cam on the lily controller,hoist was back in service 7:40am.Mine evacuated,no injuries/other problems.Service rep's called to evaluate & check programming.
A slab of roof rock fell between spad #889 & #888 in No.6 entry (return) and was detected during a weekly examination on 5/7/08. The rock measured 12 feet long, 4 feet wide and 3' to 4' thick. The area was dangered off and mine posts installed in the area. The area is outby the face.
# 3 hoist kicked on ""motor E overtemp"". Electrician was called to surface to reset the hoist. There were still 2 hoists operable at the time.
Roof fall occurred on MMU 005 inby the section tail piece. The fall was located XC13 between entry 1-0. Cause of the fall was stack rock.
An unplanned roof fall was discovered in the return air course in a room panel off 10 Right Main.This fall is located at spads #5241 and 5242. It is approximately 10 feet high and 60 feet long. The area was supported with 6 foot, fully grouted resin rods. The area was dangered off. The fall did not affect the ventilation at this mine.
A roof fall has occured in the #9 seam in the Main East in the #4 entry in part of the intersection of crosscut #15. The fall measured approximately 12'x15'x8'. The area was supported with 6' tension rebar bolts, 12 cable bolts and truss bolts.
An unintentional fall occurred In the #9 main north entry at the 18+70'. The fall was 80' X 19' X 7'. Area was mined 3/18/06.
During the weekly examination, a reportable roof fall was in Sly Gulch Return, entry 3, crosscut 1 - 2. The fall was approximately 17 feet wide, 15 feet long, and 8 feet high.
A roof fall occurred in the morning of 5/6/08 on #1 unit in the #4 entry approx 70' long, 18' wide and 5' high. During investigation it was found that a kidney bed had been encountered in the one entry only.
An unplanned roof fall occurred in the #130 mine on #7 belt, brk #12, #4 entry that measured 60' in length, 10' in height and 20' in width. The area was developed on 11/6/06. No injuries were reported.
Roof fall 3-left panel #2 entry intersection crosscut #5 outby 30',total length approx. 50', 5' in height, no equiptment involved, no one injured.
A mechanical problem occurred on the Koepe man cage. The overspeed cam that operates the overspeed contact was loose/not making good contact and would not allow the Koepe to function properly. The mine was evacuated of the 40 persons in the mine. The problem was corrected and Koepe back in service at approximately 3:30pm. No injuries; no other concerns.
Unintentional roof fall discovered this date. Location of fall is North West Mains, left of station 4667 in crosscut between 1 & 2 entries. Fall measured approximately 18' wide and 40' Long. Height is undetermined. Overburden in this area is approximately 320'. Previously supported with 6' full grout resin bolts. Area mined on or about 6/11/06.
The mine encountered a fall of roof on the 3rd MNW, XC 43+20, between entries #3 and #4. The fall measured 45'L X 19'W X 8' thick. The area was developed on 6/25/2006. The fall did not affect ventilation or passage of men, no personal injuries or equipment damage. The area was supported with 8' point anchor bolts. This fall will not be cleaned up.
A roof fall was discovered in an area of known roof conditions. The fall measured approx 60' long x 20' wide x 4'-10' thick. Fall occurred in the #7 entry (intake) at survey station # 5629. The fall had previously been supported with 10' cable bolts & wooden cribs had been set around & under the roof before it fell. Fall is located approx. 12,000' outby face of 001-0 section.
A roof fall occurred on the south section mmu 002-0 #2 entry south h b xut intersection 4 way. Fall was 18' x 18'x approx. 7' high. 6' fully grouted roof bolts. no one injured or trapped no ventilation or excapeway was comprismised.
An unplanned roof fall occurred in the #2 gas mine on #1 section in the #1 entry. The fall measured 105' in length, 18' in width and 11' in height. The area was developed on 5/5/08. no injuries were reported.
Cutting coal and rock in the No. 2 entry on the No.7 Section when a frictional methane ignition occurred.
A rock fall was discovered in the #1 entry 20 feet outby spad 534 in the 2nd sw mains. The fall was aprox 20' long x1 8' wide x 64 inches thick. The fall did not affect persons, equipment nor ventilation.

At 12:30 am the Kuhntown elevator stopped about 150' off the bottom. An employee was on the elevator at the time and was trapped from 12:30 to 2:00 am. A contactor was found to be bad and tripped the overload. The contactor was replaced, elevator inspected and put back into service after K-order was lifted.
A 30' x 18' x 4' thick roof fall occurred in No. 5 track entry one break inby spad No. 12822.
Unintentional roof fall on 012 MMU, #8 Entry, brk #10. 25' long x 15' thick x 20' wide.
The #1 elevator was removed from service for scheduled maintenance @ 9:00 am. The Bottom man way man door was removed, repaired and replaced. The elevator was returned to service @ approx 6:00 pm. Escapeways were re-routed and individuals were notified of the change during the planned outage.
# 2 hoist kicked due to a grounded trailing cable. The hoist was put in manual mode and back in service at 2:30pm. The trailing cable was then repaired and back in auto mode @ 6:30pm
Unintentional roof fall on 011 MMU, 5 Right to 6 Right, 60' long x 10' thick x 20' wide.
A roof fall occurred in 9 rooms section #5 butt 10' inby section #5297. Fall was 18' w x an undetermined height x 200' long. Fell from 3-4 entry to #7 entry. 3-4 was bolted on 3x4 with straps in the intersection & 4 8' resin bolts. 5,6,7 entries were bolted on 4x4 with 6' torque tension bolts & 4 8' resin bolts in every intersection.
#3 hoist tripped on ""spear point fault"" south srip, south spearpoint. Electrician reset hoist and it was back on service @ 0155.
#3 hoist ricked on loading pocket fault. It was determined that the hoist could be run in evacuation mode. Upon investgation it was determined that a fiber optic cable was damaged causing a communication interruption. Cable was repaired and hoist was back in auto mode at 1500.
In #1 Heading the butt off or side cut cut through into old works. Air moving from sec into old works, measured 10,200 cfm. No water. No roof problem in the cut.
A rock fall occured in the 3C Panel, left return, #10 entry, spad 24+71 to 24+03.
Miner was drilling a slab shot when the roof collapsed in on him causing fatal injuries.
The victim was walking in an outby direction on the outby side of the scoop, which was being trammed over the victim resulting in fatal crushing injuries.
EE was backing up on stockpile to dump loaed 775D rock truck on oneway stockpile. Rock & truck backed over end of stockpile. Truck flipped over backwards and landed upside down about 30' below. No damage to cab, employee suffered fatal blunt force trauma.
Based on investigation it is belived that EE climbed up on top of the pan feeder on an extec crusher while it was in operation. This was in violation of company policy & training. EE went through the crusher and was killed.

While atempting to repair a shuttle car without determining that the trailing cable was de-energized and without locking-out and tagging the power source. EE received fatal injries when he cut into an energized phase lead.
EE dug some holes in water, then the ground gave way and the excavator turner over onto side, trapping EE inside.
The man was cutting bottom with the continuous miner, when a piece of material flew back and struck the man on the safety glasses. The material damaged the glasses and embedded in his right eye.

EE was setting shield #79 on the o-Panel longwall and his right foot was caught underneath the pontoon and relay bar of the shield which initally resulted in fractures of the foot and ankle then later required amputation of the right foot.
While loading a shot, the employees right index finger was struck by a rock and needed to be surgically removed at the second knuckle.
Employee dismounted his equipment (Haul Truck) to remove a boulder in the road. Employee says he lifted a boulder that weighed approximately 25 lbs and returned to his equipment.
Employee was in the process of helping get a pin steel out of the pinner pot. Another operator was helping him. The operator did not know where he was because he was concentrating on the pin a nd chuck. For some unknown reason EE put his hand on the boom, the same time the other operator was raising the boom and caught his hand between the boom and short arm on the bolter.
Climbed up on side of crusher while it was operating, caught arm in head pulley. It dismembered arm at elbow.
While lifting a conveyor section for a new coal loading facility the ground beneath the crane subsided causing the crane to topple.
Employee was lacing up air hoses between railcars. Employee put his hand in a known pinch point area (railcar couplers). The slack rolled in and pinched the pinky, ring and middle finger on his right hand. Causing a laceration to his right ring finger that required seven stitches.
Working in steam bay the employee rolled a creeper under a D10 dozer to gain access to clean the underside of the machine with a pressure washer. While removing the creeper from under the machine, the wand slipped out of his hand and engaged the trigger on the wand. The hot water shot him underneath the face shield and burned the skin on the right side of his face and neck.
Employee's shift began at 7:00 p.m. on 5/30/2008. Employee fell off of the ladder onto a shovel during shift change. EE was released to full duty on 6/12/08. EE returned to clinic on 6/25/08 and was not allowed to return to work until further tests are performed.
While installing new plates in the F2 hopper with another co-worker, employee began to install the 2nd angle iron when he noticed welding debris in the area to place angle iron. EE leaned the angle iron slanted against the 1st angle iron to clean debris when the 2nd angle iron slipped and smashed his right hand middle and pointer fingers between the ledge and iron plate.

On 12/8/09 EE 1st saw dr and diagnosed with a strain without restriction. During layoff the EE received medical restrictions. Layoff callback occurred on 3/19/10, but EE could not return due to restrictions.
Miner operator was injured when the miner bridge was accidentally dropped on his back as he was crawling under the bridge.
Employee was performing normal activities in the logisitics area. He stood up after hooking up a hose to a railcar and felt a pain in his right side. The employee was diagnosed with a hernia. The employee did not report this to management properly.
Employee was operating a loaded water truck, the truck started skidding and the employee jumped from the truck hurting his left foot.

EE was working on the back of the operator's cab of a 785 rock truck. He lost his footing and fell about 12 feet to the ground, landing on his feet.
Employee was operating an empty haul truck traveling upgrade at approx. 10 mph and apparently veered to the right and powered up the unit causing the truck to push through the rock berm into a ditch.
Employee was helping to remove a stopping by hand. As he pulled a block out of the wall a second block fell striking employee on the hands. Employee has a fracture to his left thumb.
EE was lifting his tool box onto a ""buggy"". When he twisted to load the box he experienced pain in his groin,left work due to this pain and since then he has complained that he had pain in his lower back.
The employee was in the process of removing a stuck drill steel in the mine roof when a piece of rock fell striking right hand positioned on the drill pot.
Harrison Mine-fell down 50' slope and fell down and over four benches.
While standing beside the roof bolter in entry no. 4 on section no. 4, employee claims a piece of rock fell and struck him on the back.
The miner was trying to access the feeder. He was assisted in taking the guard off the pick breaker. As both miners were raising the guard, one miner screamed with pain.
The employee was dragging slack miner cable by hand. He tripped, falling awkwardly on his right hand. He sustained a contusion to the back of the hand and fingers. X-rays showed no broken bones.
The injured stated he was cleaning safety glasses and mandoseal got in left eye, scratching it.
Employee was installing outlet gate under railcar when outlet gate shifted on jack and pinched fingers between outlet gate and jack. Resulted in fractured little finger.
The employee was cleaning up the machinery.
While removing the kiln burner pipe using a come-a-long, the anchor point to which the come-a-long was connected failed and broke away causing EE to fall to the floor hitting his head and cutting his ear.
Employee stepped out of the operator's compartment of the scoop he was operating and slipped, twisting his knee. He began losing time on 06/11/08.
The employee was checking the end dump tailgate for material. There was a rock blocking the tailgate, keeping it from closing. He grabbed the rock with his left hand to try to loosen it. The rock came loose and the tailgate shut, pinching the tip of his left index finger and tip of left thumb. He was wearing gloves.
While tightening top of drill pot, the wrench slipped, causing EE to strike pot with his right hand.
Employee was lifting 20 lbs block when he stated his knee hurt.
He was assisting while advancing the slider tube. The tubing fell and struck him on the right shoulder causing a right shoulder separation.
This electrician laid his hand on the side of the feeder and a rock flew out and struck the back of his left hand. Injury is described as not a fracture however the bone is ""flattened.""
The condition that contributed to the accident is placing a ladder on the side of a fuel tank to get stick readings.ladder was placed in a bad location and was not stable.EE steped on the ladder to come down the tank it slipped out from under him and the employee fell. The employee fell on his shoulder and broke his clavical.
Employee was assisting another employee with a car cable that was not taking up; when he discovered the problem, the operator started his shuttle car causing the cable to start taking up. The cable caught the injured's legs causing him to experience a same level fall resulting in a fracture to his left wrist.
While starting to drill in the roof a piece of rock fell and struck the edge of the canopy which EE was standing under, the rock flipped back into the area where he was standing and struck him on the back.
Employee was removing a portable antenna from the top of the dozer when he felt a sharp pain in his lower back.
While closing rail car hooper door injured lower left part of his back.
EE was alking down the intake entry and stepped into a hole and tripped, EE caught himslef with his right leg and this caused him to twist his knee that resulted in a spain.
EE was cleaning the tip of his welding torch, by wiping it on the palm of his left hand welding glove; after several tries, the glove caught fire and burned his left hand.
Employee stated he was getting onto dozer when left foot slipped off step. He had hold of grab iron which prevented fall but it strained right shoulder. Hurting but seemed ok. Went to doctor later. Found small tear in shoulder. Opted to keep working. Finally decided on minor surgery to repair tear. Did so and lost time starting on 11/17/08, thus reportable.
EE was getting out of Prell Truck, when he slipped off step and pulled muscles in his back. EE did not go off work until 6/17/08, he came back on 6/23/08.
EE was working with a pump at RNS Services Dilltown site. Upon completion of this job He felt slight pain and discomfort in his back. The next morning He was not able to report to work due to severe pain in his back. HE did go to Miners Hospital for treatment on 5/29/08.
The injured was replacing a lid or cover back on the roof bolting machine. He strained or pulled something in his lower back.
Bent to pick up a piece of marble 51"" and felt a pull of right side abdomen causing pain on right side.
While removing a drag drum bearing cover the cover slipped causing the employee to twist his back. MRI received no disk damage. A strained nerve is the diagnosis.
While riding manbus to job site, manbus hit a dip along track, bounced injured ee's head into canopy of manbus. This pushed head downwards between shoulders. He used his right hand to cushion the fall injuring, hand, neck and head.
Employee was helping to load bolts onto the miner and after had pain in his groin area.(diagnosed with a hernia)
Miner was setting timber and a small piece of right roof rock fell striking miner.
This electrician was replacing a guard on the miner after making repairs. While holding the shield in place and getting the bolts started a pain shot through his groin & testicles.
Driver slipped from the loader platform, falling 6-7 feet and hitting the back of his head and back. Currently, the only injury found is bruising. Driver will be seeing a specialist.
Employee was loading a backhoe onto a trailer, the backhoe slid off the side and overturned. The employee continued to work until 06/26/2008, on this date the doctor took him off work to have surgey on his shoulder.
Employee was traveling up a hill when the truck spinned out and slid backwards into the berm and overturned.

EE was stacking 50 lb bags onto a pallet when he felt pain in left shoulder. The doctor believes it is just torn muscles, not a rotator cuff.

Injured was hanging ventilation tubing 8R section, 40' outby #10 break in #1 entry. He went outby the LOB to get a piece of tubing 20""x10' he bent over twisted left handed and jerk. Felt something pop and felt pain in lower back. He reported numbness in his back 20 min. later. He went to the hospital where he was treated for back pain. Therapy and medication.
EE was tramming the roof bolter up the neutral entry from the operators deck. He got out of the deck to use the inch control levers to go under a discharge line going across the entry, when the chain from the front left tire broke. A link from the chain struck EE in the left lower leg.
Employee was loading powder when debris fell hitting hie eyebrow and falling into his eye requiring medical treatment.
Employee was standing along side of a parked fork truck when a second fork truck backed around the corner hitting the employee.
While changing clothes to begin shift, something stung or bit him, causing swelling and inflammation. Three days later, on follow up appointment, hand is swollen and infected.
The employee was going down the Clarifier stairs when a bird was going to bite him. While trying to avoid it, he slipped and injured his waist and left leg.
Over-ther road hauler was delivering a load of Petr. Coke to the South Kilns storage yard during the afternoon shift on 5/27/08. While in the process of trying to unload his trailer in the raised position, a strong gust of wind combined with the wet sticking load hanging up inside the bed, caused trailer and tractor to tip over. Contusions to right arm and ribs.
An auger being hoisted with hydraulic cable hoist, slipped off hook and fell on EE's left foot, smashing his toes.
The employee stated he was supplying the roof bolt machine and when he lifted one metal strap onto the tray, he immediately felt a sharp pain in his lower back.
EE was in the process of repairing or replacing a grease hose to a pillow block bearing. The bearing was on a hold down roller on the overland ""C"" belt. The belt was running at the time. His left hand and wrist came in contact with the belt and roller, causing a laceration to his left wrist and a tear to the palm of his left hand. He was sent to local medical facility.

EE was crawling across section when he put his weight on his right arm and it buckled under him and caused him to fall onto his arm and dislocate his elbow.
While moving the continous miner on #1 section in the #5 entry. EE became pinned between the boom of the cont. miner and coal rib. EE was transported to ER where he was diagnosed with a collasped lung and contusions, x-rays revealed no broken bones and his condition is stable.

The EE allegedly strained his lower back trying to lift a scoop charger and put it in a scoop bucket.

Employee twisted right knee mounting his haul truck. Employee returned to work performing normal duties driving truck. On 6/8/2008 employee worked in shop and right knee swelled up. Employee went to doctor on 6/13/08 where he was given a restricted duty release.
Member stepped off school bus that is used to transport members from parking area to portal twisted left ankle. Referred to specialist. Specialist hold member from work.
Employee was laying on back under beltline lifting with legs. Lifted wrong.
The employee was working on the wet screen, when a peice of metal hit him on his head. He had some head pain. He was taken to Santa Paula Hospital ER for further evaluation. He was released a couple of hours later. He was fine and doctor gave him 3 days off from work.
Loose grip on grinder-grinding tack weld to finish weld crack on truck dump box, disk caught in crack, kicked grinder back, knocked PPE off, grinder struck chin, lip, nose & left side of face. 1 1/2"" cut chin to lip-10 stitches, abrasion left cheek, broken nose-will need surgery to align, many fractures to cheek bone-no further medical unless complications occur.
Moving bearing heater cart across shop floor. Top cross-bar of heater fell from cart onto his left foot.
Injured was using a 12"" crescent wrench to take and oil filter off on the continious miner. He felt a pull in his right arm bicep area. He went to the hospital where he recieved medication and time off for the strain.
Employee lifted a hydraulic ram and felt pain in his abdomen.
Employee was riding in a motor when it lost traction and hit a manbus causing the employee to his his knee against the frame.
Employee started to come down a ladder when his head bumped a beam startling him into letting go of the ladder and falling about 9 feet to the deck. He broke is left heel, bruised left elbow and knee.
Employee stated that he missed a rung while descending a step ladder and fell about three fell about two or three feet to the ground. he was examined by a dr and the x ray revealed a crack in his left heel.
Employee was struck by a four-wheeler ATV on the Leach Pad.
Employee was bringing out a 2x12 board from the cement separator and tripped on the steel toe plate around the ducting, fall on his knee.
Truck was being loaded by loader.The 2nd bucket had a rock in it,which jarred the EE and broke the windshield.The EE told his supervisor he felt fine and took the truck to the shop for windshield replacement.When he bent over to pick up the chock,he stated his lower back began to hurt.He was taken to the clinic and taken off work several days. ONGOING.
Unplugging crusher, when pry bar got caught in crusher wheels, bar then came up and hit EE under neck. Knocked EE out, chipped teeth, hit head and hurt back.
Employee stated he had been working on the ratio Feeder on this unit, when he turned and started walking away, when he struck a bent Roof bolt with his left shoulder. Person started missing the on 10/27/2008. MRI Impressions showed no defined tendon retraction, degenerative signal intensity, and the most likely diagnosis is Tendinosis. (Occup. Illness)
EE lifted the end of a 12"" x 17 foot long screw and sustained a hernia in lower left side of stomach. Surgery is scheduled for June 27th and return to work on June 30th. Currently working but on restricted duty for lifting.
miner was taking down tubing at a tubing regulator it slipped and fell on his leg.
After returning tools to a tool box, the EE turned to walk away and felt/heard a pop in his leg. EE was not performing a task or carrying anything at this time. The EE's achilles tendon was torn. Surgery was required.

While brazing fitting on 5230 hot hydraulic oil came out of a line and hit left knee causing a 2nd degree burn.
EE c/o having pain in his back when bending and lifting stone. He was stacking garden steppers & colonial at the time with Other EE, and Another ee. he then finished wraping his colonial perfect after that other ee took him home.
EE was removing an air exhaust muffler from a compressed air dryer system.After releasing the residual air from the air tank and line the EE unscrewed the valve not realizing there was still some stored pressure inside the line which blew small desiccant beads out striking the back of both hands and the back of both legs.
Employee was training on an ATV. The employee drove over 10"" pipe and mis-judged the pipe, causing her to flip over the handlebars of the ATV.
Walking back from lunch in #4 entry. Slipped in mud, fell and injured right hand.
As the EE was loading square set into the bucket of a scoop, another square set rolled off of the pile and struck him on top of his left foot.
Member was picking up trash after building stoppings. He put some of the plastic bands that hold cinder blocks together in a plaster bucket. One of the bands popped out of the bucket and went behind his safety glasses and scratched his eye.
Miner was hanging miner cable and strained lower back. He did not start missing work until 5/29/08
The electrician had locked out the breaker to the miner while performing tests. He gave the key to the operator and had him to reenergize the breaker. There was shielding wrapped around the cathead and this caused the catehead to be grounded. When the victim engaged the breaker it exploded. Burns to his hand and side of his face.
Miner was checking air velocity at vent well. Dust blew out of exhaust pipe, startled miner and he fell backwards.
Come-a-long broke loose from tailpiece and hit employee in low back toward the left hip. Employee continued to work until 6-3-08. Missed work 6-3;6-4;6-5. Did not know he was off due to the injury until 6-6-08.
Employee was moving 6"" hope pipe and caught right hand 4th digit between two pieces smashing finger at the fingernail.
Employee stated that while operating the roof bolting machine, a piece of rock fell from the mine roof striking him on the head and shoulder.
Employee pulled himself up on jumbo hood to work on slip rings and stated he pulled his back. He was seen by company dr. on 5/21/08 and again on 5/27/08. Dr took off work pending x-ray and wants to keep him off work until next evaluation.
Lack of communication: I was guiding a piece of steel down attached by the crane and a co-worker struck the steel with the hammer as I reached with hand to straighten it. He accidently hit my fingertip in the process.
EE stated that he was installing a man door, when a piece of draw rock, appx. 1""x36'x36"" fell and struck him on the back. EE continued to work until 8-8-08, when he consulted a Dr. that took him off work and started physical therapy.
On 5/21/2008 the employee was pulling cables through stopping and felt pain in left groin area. He has continued to work until 7/9/2008 when he started missing due to pain in groin area.
Employee was going over dirt slope during in lighting storm when he slipped and fell and sprained left ankle.
Employee was tramming a Mac-8 rubbered tired mantrip to the charger and struck the rib dislodging a piece of rock that struck his right hand.

EE was walking down the #2 Gallery from the #3 transfer to set an e-stop that was down. He came across two lights that were out, he reached to grab a hold on the roller frame and stepped on some gravel and slipped. His foot got caught on the beam on wall side and he twisted his knee.
Employee lost his footing on wet steps of ladder and broke his fall by grabbing on a railing. He continued to have some pain in arm and had arthroscopic surgery on 1 23/2009.
Miner was walking stepped in a hole and his hip / leg hurt. Miner did not begin missing work until April 1, 2009.
Employee was attempting to start a pump, when a falling pipe fell toward him, knocking him down.
Employee was removing garbage from a railroad car when he stepped on some loose coal, causing him to fall backwards. The employee began missing work as a result of this accident on 5/26/2008.
Employee was watering batteries on the Long Airdox Scoop. The lid was not latched and fell on employees' left forearm.
The EE started his regular shift. He got one of his fingers on his right hand stuck while unloading the bulk bags.
Pulling pin out of stone and measuring a slab, he had both feet under the stone, and stone shifted, pinning both feet.
Employee was dragging rock dust hose in #4 wall crosscut track to belt in E-17 Section and tripped on a piece of concrete block causing him to fall and strike his head against the belt stopping.
Employee was walking on uneven ground and mis-stepped twisting ankle.
EE was pumping mix in yellow bags from step ladder, when EE came off ladder, his left foot hit on uneven ground and caused left ankle to turn.
While changing a jaw crusher die, the die struck EE on the left knee, while moving the die into place for installation.
Employee stated that he slipped stepping into ramcar and twisted his left ankle.
Greasing the gates on hopper, slipped and fell against spray bars - bruised ribs.
Employee was standing on a crossflow hose hitting an 8"" pipe and slipped and fell hurting right lower leg **EMPLOYEE DID NOT START LOSING TIME UNTIL 6/3/08 DUE TO INFECTION**
EE was attempting to remove a rock from between 2 rear tires when the insode tire blew open. The EE received a fractured R. wrist, fractured index finer on R. hand., a laceration to his R. thumb (requiring 7 stitches) and a small cut to his forehead, EE was taken to ER and realeased around 2200 that night.
Employee was descending a vertical ladder attached to a haul truck that he had been servicing. Upon descent, his knee gave out and he landed hard, twisting his ankle.
EE was greasing standing on a ladder greasing a haul unit in the lube bay. He was standing on the third (3rd) rung from the top of the ladder. When he pulled on the grease line that was attached to the grease fitting, he lost his balance and fell 8 feet to the ground landing between two grease barrells.
EE was moving the breaker grid from the impact crusher. The grid struck his hand (left) cutting it.
Lifting a tube to hang, tube rolled pulling arm back. He continued to work at his regular duties until 7/15/2008 when he was removed from work for repair of a torn rotator cuff.
Employee was attempting to stuff and wedge an intermediate bolt in an overhead location. While holding/pushing the bolt, he felt a sharp pain in lower back. *Did not start losing time until 1/30/09 due to surgery.
While moving roof bolting machine, he stepped in hole and hurt foot. ** started missing on 06-02-08**
Operator was pushing shot when Dozer backed into a rock causing the dozer to stop suddenly. This sudden jar caused the operator to hurt his lower back.
EE was moving rescuers on man trip when he felt pain in his lower back. EE does have a prior back trouble.
Employee was walking down some stairs and missed the last step, rolling his left foot underneath him.
EE was shoveling wet muck at #3 transfer house on the third level when he felt a pop in his lower back on his right side and fell to the floor. He got up and tried to walk it off. He was helped down the stairs to the truck. Assesment was made and decided to immobilize on a back board and transport to Holy Cross Hospital. Supervisor called the ambulance.
The employee was walking in #4 entry of the #2 unit. He heard someone say something and turned suddenly to respond. His left leg twisted on loose material on the mine floor. He sustained a sprain on his left knee.
The injured was removing a rock from the haul road when he felt pain in his groin.
Employee was pushing rock with a Cat. D11 dozer when the blade of the dozer slipped on a rock, causing a quick jerk to the operator. Employee complained of lower back pain. He drove himself to the Mary Breckingridge Hospital in Hyden, Ky where an MRI is pending.
Helping truck driver pull 35' section of tubing from flatbed onto forks of forklift. Tubing became unbalanced & started to teeter. Employee used shoulder to push tubing back on truck. He heard a pop & felt pain & numbness in L Arm & hand. Final medical documentation for determination as to whether this was MSHA recordable wasn't received until 9/15/08.
Employee was changing bottom stands on belt line. #2 was pulling stand out from under belt. Stand got caught and then as pulling hand it came loose and the frame on stand against hand hit 4"" water line, resulting in hand being mashed between them.
Employee was driving a rock truck from the quarry to the crusher and lost control of it.
Pulling rockdust hose and felt pain in groin area was diagnosed with a hernia and his first day of lost work is 5/29/08.
Employee was working on the back end of a moving trailer when he lost his footing and fell to the ground. A Straw Blower machine attached to the back of the trailer ran over the employee's pelvic area.
Employee was getting tools out of tool box, when tool box lid fell, hitting employee on the left hand.
Employee had just signed his time card and was heading toward his vehicle. Employee crossed the scale and stepped off and twisted his ankle. The height of the scale was 18 inches.
The employee was loading rock dust from the supply car into the bucket of the unahauler, when he felt something pull in his lower back.
Injured and other ee picked up a piece of top belt structure and moved it 8' and sat it back down. Injured reported his left shoulder was hurting and may have twisted it. He went to the doctor where they treated him for a strained shoulder with medication and time off.
Employee attempted to drop out 29 empty rail cars from empty track to 40. He could not control cars - went by 40 and through crossing into load track, thus colliding with 28 loaded cars. 1) weather wet & rainy. 2) Employee did not follow policy in that he brought 29 empty cars when policy calls for a maximum of 12 cars per 1 dropper.
Accident location: 5 Butt, # 2 entry, between crosscuts #2 & #3 - Track Entry. Employee slipped and fell while performing preshift examination. Stepped on steel rail (track), boots were wet causing to slip. Fell on cap light battery on lower right of back resulting in loweer back pain.
Employee was on his knees loading bags of rock dust into a scoop bucket. A piece of coal got between his knee and knee pad he was wearing. The rubbing action of the coal against the knee caused a soft tissue injury and possible chipped knee bone. He started missing time on 05/19/2008.
Rock hung in conveyor chain flew out striking nose. Broke nose.
EE was fusing pipe together, while in the process, he injuried his knee. **This was not diagnosed until 7/23/08.
Miner walked through a line curtain and a rib roll fell onto the side of his foot.
EE was operating scoop and hit a bump, bounced up and when he came back down in seat, he hurt his lower back (tailbone).
Employee was preparing to load a used scoop tire, using pinner steel inserted in rim of tire to set tire upright when the coupler on the end of steel broke just as tire was upright, tire flipped back, employee lost balance and fell backwards, catching himself with left hand, fracturing wrist.
Member was lifting door and strained groin. Hernia will be repaired.
EE got down off the HWM and stepped in a hole, fracturing his left ankle.
Operating roof bolter in face area, when lowering roof bolter boom, hose blew-squirting oil into his left eye causing an irritation. Was wearing safety glasses with no sideshields.
A rock fell out of the top hitting the extended slider curtain knocking it down and the slider curtain struck employee on his right side.
Employee states that while walking along side a suspended load, he felt something pull in his calf muscle. He noted that the leg muscle had been ""tight"" for 2 days prior. There was no specific incident that he can attribute the pain to.
While walking along the operators side of the roof bolter, EE claims to have stepped on a small rock laying along the rib, as he stepped on the rock EE lost his footing and fell to the mine floor.
EE was cleaning a chute. EE slipped when EE stepped down, fully extending EE's leg with all EE's weight on it. EE felt a popping EE's knee when walking.
Put his hand in tray on roof bolter and a small piece of slate hit his finger.
Employee picked up guard by ring (washer) that was welder on guard. Ring broke loose when removing guard-fell on his left hand
Employee was attempting to rotate the shaft, using the overhead crane, he was controlling the overhead, pressed the control button passed detent and the hoist raised too fast. The shaft came up and rolled in the direction of the employee pinching his right foot against the exciter set frame work. Transported to clinic, examined, and unable to return for his next scheduled shift.
EE was working on the Telsmith #44 tightening bolts. Wrench slipped off the bolt causing him to fall into the belt guard and motor. He received a scraped shin and buttocks. He did not go to the doctor until 5/20/08.
While dismounting from the trackhoe, he strained his lower abdomen.
Was lifting a walk way to work boat and strained lower back.
Employee was cleaning spillage in outside tunnel when a belt splice caught his clothing pulling him onto the #5 belt and he was then caught by the belt chute for the outside slate belt. He received contusions and lacerations to his upper back, chest, legs and arms.
Operator was operating forklift & came in contact with a room support column; stated that ""he looked down for just a second"" thinking he was past the column.
The employee dismounted his scoop feeling sharp pain in lower back radiating down his (L) leg.
The man was lifting a roll of conveyor belt when he felt a pain in his right shoulder.
was driving pin (with hammer) into excavator tracks. part of pin chipped and part of pin went in left inside leg...resulting in treatment of antibiotics, tetanus shot, one stitch, and general cleaning...missed one day on 5/14/08...returned to work 5/15/08
Member was shoveling material away from tailpiece and through a door to outside as he threw a shovel full of material he felt pain in hiw lower right side below his belt he continued to work until 5/22/08 when a swollen lump developed on his abdomen he went to the dr and was diagnosed with a hernia and is scheduled for surgery on 5/22/08.
Employee was welding on drag line bucket. While standing on a cable spool he attempted to step off the spool, fell to the floor and landed on his left arm. He was transported to a local hospital and diagnosed with a broken left wrist.
EE was untangling miner cable, when he pulled on the cable and felt pain in his back. Pulled muscle in his lower back.
Co-worker and injured were instructed to drill a test hole to evaluate the roof. The injured was unaware the co-worker was ready to tram the roof bolt machine. The injured was reaching for the drill steel stored on top of the machine when the co-worker trammed the machine forward. The injured sustained a contusion to the top of his right foot.
Employee reported: Had set up the miner in the face of No.3 entry, Hung his wing curtain and was standing alongside the machine's tail. A piece of draw rock fell, partially caught by the wing curtain and flipped over onto his head and shoulder, sliding down his back.
The excavator operator was moving his machine to refuel from the service truck. The employee operating the service truck was standing beside the service truck and the excavator operator accidently steered the excavator quickly toward the service truck striking the service truck operator with the bucket of the excavator.
Miner was breaking down a granite block and there was a crack that did an oval around the back of the block when the block broke down the block slid into the mine and the miner followed.
Employee was trying to clear a crusher, when the bar he was using, slipped and caught his left forefinger between the bar and the side of the crusher. The metal ripped the meat from the finger at the second joint and broke his finger.
A piece of refrafractory fell off a wall and hit employee while he was cleaning up debris. He was knocked into the wall and injured his right shoulder and was cut about his eye.
EE slipped on a rock and sprained his ankle.
Injured party was in the process of moving a pump hose when he nicked the hose with his left foot. As he did this, his right foot slipped and caused him to fall, hurting his right knee.
Jacking on a flat car back on rail and bar slipped and hit employee in shoulders and jaw.
EE was kneeling on top of the #19CH. He then stepped down to the mine floor when his foot came down on a rock about the size of a baseball. His ankle bent one way and his knee went the other.
EE was traveling down steep grade, when the truck apparently came out of gear and he lost control of the truck. EE jumped out of the truck, causing scraps and bruises to himself.

On 5/12/08 the injured was hanging a roof bolter cable and felt a pain in his shoulder. He continued to work until 7/21 at which time he received outpatient surgery. Not reportable until 5/12/08.
HE WAS WALKING and HE STEPPED IN RUT, CAUSING HIM TO FALL, TWISTING RT. KNEE AND ANKLE. ** STARTED MISSING 07-23-08**
Employee was at the top of the stairwell, was attempting to come down when he missed the first step, falling down 7 steps.
Employee was changing a conveyor belt in the clinker storage area. The employee was exposed to clinker dust for an extended period causing irritation to eyes, skin and throat. He had a burn from the dust to the ankle and his eyes and throat were irritated causing discomfort.
While EE was tramming the miner, the EE reached next to him to remove a ventalation curtain and struck his left foot with the bull wheels of the miner, causing a fracture to his left ankle.
Employee was setting roof supports in addition to permanent supports, when a rock fell & struck the victim's left arm, causing him to fall & also strike his head on the rock.
While working on hammermill, got cut on right hand. No accident report was filed. When injury healed, he realized there was metal stuck in his hand. He had surgery to remove it on 6/25/08 and was off work until 6/30/08. Missed 3 days of work.
EE moved to the next shield slipped and fell backwards onto his back.EE began missing work on 5/22/2008.
Employee was loading rock dust into a duster. He was bending over and lifting the rock dust bags from the mine floor. He experienced pain in his lower back. He continued to work until May 19th. At this time he started to miss work.
Employee was operating #9 locomotive. He stopped the locomotive and stepped off the locomotive onto the clear and space along the track. He twisted his right ankle and fell down. The learance space was free of obstructions and had loose slag in the travelway.
Rock fell from between two bolts striking him on the head and knocking him down.
Employee was at fuel pump fueling his truck. He stepped in some loose material and foot slid into a pothole. He turned/sprained his ankle.
Employee was walking along the E-17 longwall stageloader carrying a spray bar jack when he stepped on a 42mm chain and felt a pinch on the top of his right foot. He continued to work until 5/17/08 when he began missing work.
Employee was repositioning a valve in the thickner. He twisted his upper body, but his left foot became stuck in mud and he sustained a strained left knee.
EE was loading coal trucks. He got out of his loader and the door slammed shut pinching his finger.
While sliding a bolt bin into place, employee felt a pop in his knee. He sought medical attention and was released full duty, however he return to the doctor and had surgery to repair cartilage on 7/29/08. We became aware of him being off due to this injury on 8/20/08. He is currently off work.
Operator of the #5 D8 dozer was pushing at slope material on the 1-1x slope. While backing up to make another push, contact was made with an unseen boulder causing jarring movement in the dozer which caused neck pain.
Two workers were moving a stone by hand. IW tripped on a small rock on the ground, fell down and dropped the stone causing co-worker to drop it as well. The stone then fell on him causing his injury.
As the rubber-tired vehicle was getting off the track, it hit a bump and the injured person grabbed onto the side of the frame. He caught his fingers between the vehicle and the track rail. Cause was safety awarness. Compliance with rules/regs not a factor, protective gloves were being worn, miner proficiency a factor, mining equipment/systems could be a factor.
Failure to set parking brake on truck and block vehicle.
Sampling coal at Belt discharge. Large lump hit shovel and stoved right shoulder and wrists.
Employee stated he was standing at the side and end of a ram car shoveling some rock dust out of the bed of the car. The stinger, liked a little pushing all the rock dust out, so he was shoveling the rest. The operator of the car told him to watch out he was going to do something and thinking the injured would move, he let the bed down and the injureds foot was under the edge.
Injured reported he had injured his back while lifting belt structure and refused to fill out a accident report on 5/10/08. On 05/16/08 he reported that his back was hurting and went out at 9:35 PM with back pain again refusing to fill out a accident report. He went to Welch hospital and was treated for a strained back. They gave medication and time off.
Slipped and fell on hydracone platform. He was not wearing fall protection. There were handrails around the platform. He was only 2-3 feet from platform.
EE was picking up old conduit on the ground floor of the screening plant. She was taking old pieces of conduit to a dumpster, her feet became entangled in wiring that was hanging out of the end of the conduit. She fell to the ground, injuring her back.
The employee stated he twisted his back when throwing belt structure off of flat car.
Employee was installing ground control mats when the jackleg drill jumped forward, pinching his left hand ring finger with the machine, causing a fracture to the tip of the finger and a laceration which required sutures. (misc mail 5/30/08 #4-He had complaication with the finger & dr. took him off work on 5/22. he received a full release on 5/27/08 after missing 3 days work)
Collison with concrete bridge w/pit truck #210 causing a laceration to head and cervical strain.
EE was driving ram car, as he got to the w301 air doors he started to get up off his seat in order to get off the car and as he did so, he jammed his neck on a fan switch which was directly over him.
Employee was lifting samples off of sampling platform onto truck from ladder. Left boot got stuck on ladder while turning to put samples onto truck and twisted left knee.
Employee fell from the motor-grader while cleaning the windows. Employee struck his head on the ground, resulting into a laceration and required some sutures.
He was unloading supplies when he felt pain in lower back, pulled muscle.
Injured was loading 8' bolts into the scoop bucket and one of the bands broke and scattered the bolts. He shut the scoop off, got out and dropped the bucket. As he was reaching under the bucket to get a bolt, the bucket came down on his left hand smashing it.
Employee was sweeping area near truck drop (pit). He stepped on a 2"" - 4"" piece of humate, which caused him to fall one knee and twist his ankle.
Employee jumped off of truck being tarped and left arm got entangled in life line attached to safety harness.
Employee was unplugging a drain underneath flotation cells. Water mixture from the drain splashed into the employee's boot causing a burn to the employee's leg and foot.
The employee was installing a rib bolt when the rib broke and fell and struck the employee on the leg.
Employee was n the process of extending line curtain when a section of roof fell measuring 7.5'L x 6'W x 0-14"" thick from the unbolted face and extending approximately 16"" past last row of bolts to where he was standing under a permanently supported area striking him across both legs resulting in fracture to left big toe and tailbone.
Breaking lumps of ore to put into the air lift, he reached down to pick one up when another one rolled down the slope smashing his right hand between two lumps of ore. Hand is bruised and swollen.
End dump truck driver was being loaded by loader at rock stripping site. The first bucket from the loader contained a large boulder and was dumped into his truck, jarring the truck driver. The EE's back was jarred and his back was sore and his leg went numb.
Employee was in the designated parking area to start his shift. Nelson Bros was blasting & fly rock came across the spoil. A rock hit employee on the right thigh.
Hoisting roller and chain released from hook point and hook struck employee in the face.
The EE injured his hand with the end of the hammer while cleaning the return rollers of the converyor.
EE was climbing down from loader and twisted knee.
Miner was sitting in cab of haul truck being loaded with wheel loader. The load in the bucket was extremely wet and released from bucket as a large mass causing truck to jerk and rock excessively. This resulted in an acute right mid-back strain.
Employee was on ladder, topping off crib with wooden wedges, the wedge dislodged a rock from the roof and hit him in the shoulder and collar bone. HG 12 West, 2 entry x-cut 30.
Miner was bitten on tip of right index finger by small sidewinder snake.
Slipped on wet bentonite while dumping a wheelbarrow. Traumatized back.
The employee was cutting a bolt out of a bearing housing using a 10 foot long burning bar. As he withdrew the bar, flame blew out of the hole and burned his arms and face.
Operator was spotting a bolt. Trying to get a wrench connected up with the steel and drill pot. He thinks he hit the rotation when he had everything connected.
Member was setting timbers. While setting a timber, one of the previously set timbers became dislodged and struck him on the top of the head causing pain to the head, neck, & back. No accident report was filed with Marfork Safety Dept until 5/15/08.
EE was putting anti-freeze into the radiator of a D-11 dozer. He was standing on the dozer cowl platform when he stepped down, slipping on the track pad on the dozer and fell to the ground, causing a laceration to his right elbow, a twisted knee and a sore back.
Employee fell from the service truck and landed on his neck/upper back.
EE was cutting conveyor belting with a utility knife. The knife slipped and cut his right little and middle fingers. Stitches required.
EE was being loaded at the shovel.Last dipper opened and landed on dovetail,jarring EE. She was taken to the clinic for back and hand pain, diagnosed with cerival strain and released to her next shift 5/11. On 5/12 she called in saying she was going back to the doctor on the 14th. She was taken off work until 5/22.
Employee was attempting to unload 400 lb equipment out of the bucket of the loader. As the Hotsey (equipment) was coming out of the bucket, employee slipped on the ice, and the equipment landed on left foot, pinning it to the floor. Got boot off, could see indention and bruise. Concerned about fracture.
EE was crawling back to ATRS controls on bolter and twisted his left knee.
Injured employee was removing the chute from a coal dryer and twisted his right knee.
Employee was walking in a low area of the mine and hit his head against the roof.
Employee was doing maintenance on crusher and was climbing down when he slipped and fell a few feet to the ground, landing on his back against a steel rail.
The EE stated he was walking out by in the #7 Entry 86+C Right Main Section, slipped and twisted right ankle.
Employee was laying on his side & picked up a crib block, placed it against the idler on a D-10 Dozer. He rolled over onto his back to get up then felt a sharp pain in his neck and shoulders, then running down his spine. Suffered headache and nausea.
Knee popped when walking up the stairs. Not reportable until 7/26/08 when surgery was performed.
Location: Mains. Employee was welding on the lug of a miner, chipping away from the weld. A piece of metal flew up and went into his left eye. He stated he did have his glasses on.
EE came in at start of shift complaining of shoulder pain, I asked him if he knew what happened and EE said ""No"", but it had been bothering him for a couple of days. He said he was up all night hurting, I told him he had better get it checked out.
While riding to section on Battery Powered ride, man was riding backwards and not looking where he was going. Struck his hardhat on a roof bolt, causing pains to right side of head and neck.
Employee was stepping out of a water truck. When she stepped to the ground her left foot rolled to the inside and caused injury to her ankle. Employee was new to the piece of equipemnt and not used to the step height.
The employee was building a stopping in #7 entry when a piece of rock fell from the top hitting him in the right side of head and shoulder. It caused him to fall on a block. When he landed he landed on the block and it cracked his sternum. The rock was about a 2' x 2' x 2"" thick.
Employee was removing liner plates from ball mill and hit a plate with hammer and slipped and it splintered and a piece of steel went into his left leg about 3"" deep about the size of a 22 shot close to an artery. He had another little piece in his right thigh.
EE was putting belts on FD101 screen in the crusher, when he felt a pull in his somach and groin area. EE restricted to no overtime.
The man was performing general labor duties, when he reported a pain in his left shoulder. He stated he did not know the cause of the injury.
EE felt pain in his lower back while loading roof nolts onto a bolt machine.
miner was lifting a battery and felt pain in lower back. miner did not begin missing work until 5/12/2008.
EE was leaning into a 52"" tall box, to remove a remaining scrap of material. the sideflap was standing straight up and there were 2 boxes directly in front of the box. The forklift driver put the forks into the skid and upon lifting it, shifted the box next to it, shifting the box. Because of the flap, the driver did not see EE in the box. He jumped and fell against railing.
Employee was pulling on a piece of plastic curtain when he slipped on a rock twisting his left knee. This did not become reportable until 5-12-08.
Employee struck himself in the face with the end of a scale bar, when he was hand scaling.
Employee was hanging pipe and turned around and he was struck in the left eye with a plastic tie.
Employee suffered injury on 5/6/08, but did not miss until 7/31/08. He had surgery on his knee yesterday 7/31/08.
Employee ws preparing work area to load 2500 lb. bags of bentonite when he noticed that the hopper gate was closed. Employee jumped down from deck to ground (approx. 3') landing on his feet. Employee states he felt a ""pop"".
The employee was exiting out of a tractor. As he stepped down onto the ground he stepped onto a rock that was embedded in the ground. This caused him to sprain his right knee. He was taken to the doctor's office and was diagnosed with an acute right knee ligament sprain. He was unable to report to his next shift.
Injured was operating a shield carrier when he ran over a rail. This caused injured to be jostled around in the cab and strike his head and resulted in a neck strain.
EE was taking tubing down in #3 to #4 break. Was on the 2 step of the ladder when a piece of tubing fell catching her tool pouch knocking her off the ladder.EE fell on her right side causing pain to her right hip and leg.EE went to the hospital.X rays showed no fractures.Gave pain medication and sent home for a few days off.
EE was walking behind the Launch Vehicle and around the stacker belt. When he stepped up onto the berm, to get around the stacker, he lost his footing. This resulted in him falling down the spoil slope and tumbling approximately 40 feet until he came to a stop on a stump and some logs at the bottom. EE received abrasions and contusions on his upper torso.
As Contractor backed his tool truck, he struck the bucket of the Excavator. Injured EE was located next to the bucket and was pinched between the service truck and the 345B bucket. EE was transported by EMS to the Hospital. The injury resulted in a completely serparated Pelvis.
While moving across the unit, a piece of rock (3'x1'x1""thick) fell out between pins and struck the EE on the back and left hand. He returned to work the next 2 days and then began missing work.
The employee stated he was pulling a pump cable through a cross cut and slipped and fell and landed on his left hand.
EE on #1 belt was working with another EE, the victum was in a pinch point area and was struck by battery operated 4-wheeler, pinning EE against Rib and mantrip.
Injured was traveling #1 entry in #1 monkey butt and stepped in a rut made by a scoop twisting his right knee. The hole was dry and clear. Injured went to the hospital. He was released but I have not been able to get in touch with him at this time.
Employee was pulling cable off reel and strained/sprained upper back.
A mechanic was working underneath a service truck when the truck rolled backwards onto the mechanic; coming to rest on his upper right arm. A D-11 dozer was used to push the truck forward off of the mechanic. The mechanic was transported to San Juan Regional via Aircare 1 where he is undergoing observation. Preliminary indications are that he sustained no serious injury.
Member was unplugging chute with a pipe and a hammer. As he was pounding on pipe, the hammer struck his hand. Injury resulted in laceration and fracture.
Two articulated haul trucks side swiped each other while passing.
While walking from the miner to the 15A dinner hole, the employee states that he stepped on a rock, twisted his right ankle and fell.
First load of material was dropped on canopy causing material to spill all over. Truck was tossed about as it was hit by loader bucket. Truck operator tossed around violently. Doctor stating off work until seen by own physician.
Member was in the process of putting oil in hyd. tank of D-10 dozer. He was standing on tracks of dozer lifting a 5 gallon can of hyd. oil on to the landing for the operator's compartment. His feet slipped causing him to fall backwards against the grab handle.

The injured was brushing float dust from the outside rail of the belt structure, the wire brush he was using came in contact with the outside barrel of a top belt roller, causing the wooden handle to strike his left index finger. The injured received three sutures to close the wound.
EE was stepping down from a motor to move a bus out of the way, when he stepped on slate or coal and twisted his left ankle causing a torn ligament in his left ankle.
Employee stated that he was bending a 6' bolt and felt a pain in the center of his back. He continued to work until 6-6-08 (Friday). Returned to work on 6-9-08 (Monday) with a doctors slip from 6-6 through 6-9. Operator was made aware of the day off on 6-9-08.
Wash down in crushers, 2"" water hose hit him in the knee after the fitting came off the hose. At time of initial exam, employee was released to return to work. In a follow-up appointment it was determined there was a tear to the ligaments in his knee. Surgery was performed on 7/15/2008. The employee worked up until the time of surgery.
Employee states he was installing a u-bolt style pipe hanger and as he raised it in place he felt a pain in his shoulder. Employee did not miss work until 5-10-08 due to his regular work schedule.
EE was loading 1/4""x2'x4 steel plates onto a fletcher jeep at the East finely portal bottomw and felt a pain in his lower left abdomen, EE began missing work on 5/6/08 and has been diagnosed with a hernia.
Subject was taking a tire in for the mantrip, raised head up to catch the tire that was falling and caught his head in the roof.
Employee was pulling rail when it caught on the track and a car on the track. Tension was still on the wire rope that was pulling the rail. The employee used a pry bar to free the rail. When he did, the rail moved a few inches and forced the pry bar toward the car with the employee's right hand between the pry bar and the shelter that was on the car. EE worked until 5/12/08.
Employee was bolting top when drill steel hung in the top, fell and hit left middle finger.
Went to plug in shuttle car cat head up to power center, something shorted out and caused a flash and burned EE's right hand.
The injured man lost his footing and caught himself with his left hand to prevent a fall, but in the process, his left shoulder was dislocated. He missed the next work day and then he is expected to be on restricted duty for two weeks.
EE was walking on the dredge deck and stepped on a H-Beam. His foot slipped off, rolling his right ankle laterally, causing pain in ankle.
Timber bounced off pallet and struck employee on top of left foot **EMPLOYEE DID NOT START LOSING TIME UNTIL 5/5/08**
Employee attempted to dismount the dozer when his foot slipped and he fell off the platform resulting in minor lacerations to his face and a strained right hand.
Employee was making up bolts 2 rosws back from unsupported top when a piece of draw rock measuring approx. 12"" wide by 24"" long and 3"" thick fell from between bolts and hit him in the left shoulder leaving a cut about 3 inches long.
Employees back started to hurt prior to him coming to work on 05-02-08 due to a re-occurrance of an old injury which occurred on 02-12-08.
Madisonville Tire & Retread employee. Tire changer had positioned himself so he could dismount & mount both rear tires. As he was re-mounting the outside tire, he swung it around with the hydraulic hand. He pulled the wrong hydraulic lever which brought the tire in and mashed him against his own truck.
Driver came down hill into pit with truck loaded. Was going too fast and went around slight corner, tipping truck onto drivers side.
Employee was stockpiling sand with a 980 loader. He felt a pain his back. He reported to his supervisor but did not want medical help. On 5/12/08 he went to the hospital with back pains. He was placed off work. Our Insurance carrier investigated to make sure it was a work comp. case. GRSG had personal changes and overlooked the time to file.
Employee was in loader bucket working on stacker (or conveyor) & wind blew(60 mph in this area that day) stacker/conveyor into our employee. He was pinned between stacker and loader. Loader was than backed up to relieve pressure on him.
EE stated he climbed over a rail on the dredge/float in order to grease a pulley, when stepping back over rail onto walkway, EE stated he felt a twing of pain in left knee. 5/5/08 EE was seen by Dr. and was told he a strain/sprain to left knee, gave EE medicine. EE started missing work on 5/7/08.
Operator was working a D-11 dozer near the highwall when a rock fell from the highwall and came through window in the cab door striking him on the left ankle. He was transported to the hospital where a splint was applied - bruising & swelling, nothing broken.
The employee was climbing down off a blade, when he slipped on mud on the step. He tried to catch himself on a handrail and stetched and strained his left shoulder. The pain persisted and on 10/20/08 he went for surgery on his shoulder and missed his first shift on that day.
2 mechanics were hoisting an 8 in. flanged fitting of discharge hose to re-attach it to a pump that had been repaired. One bolt was put into the flanged fitting to and secured in place with a nut to hold it in place. Due to the tension on the hose, the bolt popped loose causing the hose to break free hitting the mechanic in the lower lip causing a laceration.
EE was operating a D11R dozer, pushing rock with the left corner bit of the blade. The section he was pushing broke off of the rock, causing the dozer to jerk forward, at the same time throwing EE forward and then back into the seat.
EE popped his left knee while lifting his lunch box, EE complained of pain and swelling.
Employee stated that as he was positioning the roof bolter the boom canopy struck a piece of rock causing it to fall striking him in the leg. The rock was appx. 15 inches x 15 inches x 4 inches thick. He continued to work until 5-10-08 when he was diagnosed with a blood clot.
EE was attempting to knock a bit out of a SHM cutter head with a ball peen hammer. As he missed, he struck himself on the left thumb. He had some light swelling and discoloration. We applied ice and advised medical attention.
Mechanic was moving a full 55 gallon drum on to pallet. When he lifted the drum his neck snapped and he felt pain in his back.
Fell on shoulder while moving a water pump. X-ray showed no sign of damage.
2B section. EE was walking and slipped on bologna skin (fan exhaust tubing) and strained his left knoee.
REPORTABLE ONLY. NO LOST TIME Installing rib pin. Finger caught between pin and drill head. Fractured right 4th finger.
Injured was operating scoop when he ran over rock causing injured to hit back and hip against SCSR.
Employee was jacking trailer off the ball-hitch was stuck on ball-when hitch release it smashed right index finger between the hitch on the trailer and the military hitch on truck. He worked 5/2/08 and is currently off work, effective 5/5/08. Note: currently employee tn shop-delivery to huntsville al quarry.
EE was working on main belt, 2 other EE's were working in above crusher. A 2x8x3 board was being used to prop up a hydraulic cyclinder on backhoe, cyclinder was moved and board fell thru crusher striking EE's hard hat and head.
Employee was drilling hole in pit with air track drill and reached down to turn drill bit to see if air was coming out. The bit caught his first, middle and ring fingers cutting and removing parts of each finger.
