A roof fall was discovered in the No. 3 entry near spad location 9130. The fall measured approx. 20' long and 3' wide. No injuries or damage to equipment resulted from this occurence.
The Cedar Creek Elevator was out of service from 11:00 pm until 4:00 am due to a faulty auxilary on main contactor.
During the weekly examination of the North Parallel Mains aircourse, a roof fall was found in the #2 Entry at Spad 21286. The dimensions are 20' wide X 20' long X 4' high. The affected area was dangered off.
A roof fall has occurred on the intake airway off of #1 section at spad # 4646. The fall measured 20 feet wide, 30 feet long, and 6 feet thick. Persons nor equipment were affected. Ventilation was affected. Employees were removed to the surface. A brattice line was moved over and another intake was established. This area has been dangered off.
An unintentional roof fall has occurred on the Main East in the #0 entry between the 23 and 24 xc. The fall was approximately 35 feet in length and 17 feet in width and 8.5 feet high. The fall was timbered and dangered out.
An unintentional roof fall has occurred on the 2nd Main South at the #10-xc in the #3 entry. The fall was approximately 35 in length and 11.6 feet in Height and in the center of the crosscut extended out to 20 feet. Entry widths ranged from 16 feet wide to 18 feet wide. The fall had 8 foot cable bolts installed as supplemental support and 5 foot double-lock as primary support.
An unintentional roof fall has occurred on the Main East at the 23xc in the number 1 entry. The fall was approximately 17.6 feet wide and 12 foot high and 85 feet in length. The fall was bolted with 10 foot cable bolts and 5 foot double lock. The fall was timbered and dangered out.l
An unintentional roof fall has occurred at the 2nd Main East in the #6 entry in between the #10 and 11xc. The fall was approximately 35 feet in length and 16 feet wide and 7.6 feet high. The fall was supported with 5 foot double lock bolts and had floor to roof support in the form of Jacks and Beams.
An unplanned roof fall occurred at #1 break inby spad 316 that measured approx. 250' L X 20' W X 9-12' H. No injuries were reported. The area was developed on 12-11-07.
Roof Fall between #6 &7 entry 1ST NW Mains spad no 318& 319 fall was found by MSHA inspector and our examiner, it will be dangered off. Fall is 30' Long x 18 ' wide x5' High. Area will not be rehabilitated at this time.
On Aug. 29th at 01:15 am the dispatcher informed personel that the elevator was down with a crew of men inside it. A member of that crew had leaned into and pushed in the e-stop. MHSA and the WVOMHST was notified that the elevator was running at 02:10 am.
The elevator was down at the Camp Creek Portal for more than 30 minutes due to a bad relay on door. The relay was repaired and the elevator was put back into service.
A ROOF FALL WAS FOUND IN THE SMOOT CREEK BLEEDER IN THE NUMBER THREE (3) ENTRY. THE FALL WAS THREE (3) BREAKS LONG EIGHTEEN (18) FEET WIDE AND APPROXIMATELY SIX (6) FEET THICK. THE VENTILATION AND TRAVEL WAS NOT AFFECTED. THE AREA WAS DANDERED OFF AND PROPS WERE SET AROUND THE FALL AREA.
A roof fall occurred in the #11 seam 2-54 supply road at xc 45 -47. (1st NE Mains) The fall measured approximately 120'L X 19'W X 12'H. The fall was reported to the MSHA hotline at 10:57 am.
Area worked for awhile and then fell, approx 12' high and 100' long in #3 belt entry at spad #6840. No equipment was involved and no interruption in ventilation.
An unintentional fall of the mine roof was discovered at the #65 cross cut along the longwall panel. The fall was approximately 80' long and 4-12' high. The fall was immediately reported by phone.
An unintentional roof fall occurred on I-Butt belt line. The fall was in the #3 entry. The fall started at SS #11057 and traveled inby approximately 120 feet. The fall was 20 feet wide and 0 - 10 feet high.
A roof fall was discovered at 273+70 in a return entry near Blake's Ridge Portal. Dimensions measured 75' long and was above the anchorage.
#3 hoist tripped on drive convertor A on startup.
Rock Fall in the #4 Entry of the 1st West Main by the 19th crosscut in old works, top was laminated and moist.
An unplanned roof fall occured at cross cut #7, entry #5 of the 2nd Northwest Sub Main, off the 1st Southwest Sub Main. The fall was approximately 35' long X 17"" X 10' thick. The area was roof bolted with 8' and 10' roof bolts.
A rock fall occurred on the 1A belt between #29 and #30 break in the #4 entry inby spad #530. The size of the fall was estimated to be 20'-25'L x 18'-20'W x 5'-6'H. The area had been supported with 4' fully grouted resin bolts. No injuries, ventilation was not interrupted, belt line was caught. Fall will be cleaned up.
A roof fall measuring 18' x 20' x 5' occurred 500' outby the No.1 face near spad No.399. *The fall will not be cleaned up.
Breaker tripped at Enon Sub Station while elevator was ascending at plesant grove portal. Power was lost to elevator causing four employees on board to be stuck on elevator for more than thirty minutes.
A piece of metal broke and caught the bottom door hanger. The elevator was pulled to the surface via a manual assist. The door was repaired and put back into service.
An unplanned roof fall occurred in the #2 gas mine 1 brk inby spad #1112 in the #2 entry at brk #24 that measured approximately 20' long x 20' wide x 12' high. The area was developed on 11-16-2008. No injuries were reported.
Material fall 18' W X 35' L X 5-6' T.At Crosscut between # 3 and # 4 entry at brk 11 on 1A belt.
A rock fall occured at #5 2nd East, crosscut between 2 and 3 entries, spad 13+22. The fall is approximatley 30' long, 20' wide and 7' high.
Elevator was shut down due to problems with the counter balance chain.
NO INJURY -- While making weekly exam, a rock fall was discovered in #2 x-cut, #7 entry on 2nd left room in return. Air movement or travel way was not affected. The fall was 1"" to 4"" thick, 18' wide, and 50' long.
Fall was found at 47 break track entry spad # 851 #5 entry. Roof fell at 47 break to 48 break 60'L, 6'H, 18'W. Roof support was 6'TT bolts.
A roof fall was discovered in the #1 Entry, #1 Belt, Break 22, Spad 105. The fall measured approximately 20' long x 20' wide x 6' high. No injuries resulted from this fall.
An unplanned roof fall occured In the #6 entry of #2 conveyor belt at spad # 350. The fall was approximately 20'x18'x7'. No injuries or equipment was involved in the fall.
While baling hay in the D1 reclamation area, a roll of hay caught on fire inside the baler, when it was kicked out, it caused about 7 acres of hay meadow and about 7 to 8 acres of an adjacent pine plantation totaling about 14 to 15 acres to burn. The fire never affected the mining operation and did not leave Luminant property. The fire was contained in about 1 hr and 30 minutes.
A malfunction occurred with the lower limit switch for the access cage. In trying to remedy the problem with controls on manual which caused the lily controls got ""out of sync"". The mine was evacuated until repairs made. No injuries; no other problems. Access cage back in service at approximately 8:15 pm.
Roof fall was found at break 38 on west mains 3 breaks inby spad #1240, between #6 and #8 entry. Fall was 17.5 W x 8' H x 70' L .
An unintentional roof fall had occurred in the intake (primary escapeway) on the 4th southwest mains, #7 entry, 3600' from the 001 section, spad # 5458 . The area was dangered off, and breaker timbers were installed. The stopping line was moved over into the #6 intake entry and the lifeline was re-routed to allow for a secure escapeway.
An unintentional roof fall was found in our M4 Main, Entry #1, Cross Cut #75. This fall is located in one of our return air courses. The fall did not cause injury to miners, affect passage of miners, and did not affect ventilation. The fall measured 18'X20'X10'high.
A roof fall was found at 4 seam mains at break 10 in the #1 entry, the fall was approx. 7'Thick x 30'long x 16' Wide. No ventilation or machinery effected. The area has been timbered and roped off.
A rockburst occurred with dayshift's round at blasting time. It was discovered by the afternoon shift at 4:30p.m. A portion of the back came out. It measured 4' W X 11' L X 5' H. Bolts were sheered off in the back. The bolting was very good in that stope.
There was a roof fall at crosscut 11, #1-2nd roadway (length 35' , width 30' , thickness 8-10 ft. ) 72'' tension rebar pins were used in this intersection.
An unplanned roof fall occurred in the #2 gas mine 1 brk inby spad #1169 in the #3 entry at brk #27 that measured approximately 20' long x 20' wide x 15' high. No injuries were reported. The area was developed on 12-8-2008.
The mine roof fell above the anchorage of the roof bolts in the intersection on old # G-7 Section at cross cut #7 in the #2 entry.
A roof fall was discovered in the #1 entry approximately 8,000 feet outby the section at survey station #875. The fall measured 16' wide x 35' long x 7' thick. There were no injuries or equipment involved.
An unplanned roof fall occurred along the #4 belt at break #30 in the intersection of the #3 entry 1 break out by spad #1043.
A brow fell and it was identified by employees during the shift. The fall damaged one side of a conveyor and discharge line for the pumps. No-one was injured and no-one was around when fall occurred.
A roof fall occured at #11 1st West main, crosscut between #6 and #7 entries, spad 33+60. The fall is approximatley 100' long, 20' wide and 7' high.

An unplanned roof fall occurred in the coalburg mine at #51 bck at 17 belt rear spad 669 that measured 30'long X 20'wide X 4'high. No injuries were reported. The area was developed on 10/9/08
A roof fall was discovered in the No. 5 intake entry near spad location 1685. The fall measured approximately 30' long x 20' wide and 5' thick. No injuries or damage to equipment resulted from this occurence.
A unplanned roof fall occurred approximately 2000' from active in the #2 entry left return at spad #4645. The fall was approximately 90'x20'x5-8', no injuries or equpment were involved in the fall.
A six inch crack was discovered in number 8 heading on 11 section when methane was discovered. MSHA was immediately notified (5:07p).
A roof fall occurred in the 3rd South Sub-main, between spads # 0+00 & 0+70, entry # 8. The fall measures 20' L x 20' W x 8' H.
An unplanned roof fall occurred 2,200 feet from the active face in #5 entry SS #46108. The fall was appx. 25'x20'x9'. There were no injuries or equipment involved in the fall.
An unplanned roof fall occurred in the track entry, one crosscut inby spad # 0404, in the # 6 break on 2a belt line. The fall measured 20'Long X 12'Wide X 1.5' in Height. No accidents were reported.
SOBO was cycling rapidly, North skip loaded, hoist rolled, E-stop activated, Hoist in track limit.
The area of the fall had been bolted with 4' glue bolts, a rider seam 5' to 6' high let loose allowing a section of roof to fall. Several cribs were set in the area.
An unintentional roof fall was discovered at 10:45pm. The fall was located 40 feet inby S.S. #11,163 of the 3rd right rooms, #3 entry. The fall wars located in a intersection, and a cross cut. The fall was about 60 feet wide, 40 feet wide, 40 feet long and 8 feet high.
A roof fall occurred on the 1 North #2 Belt, in the #5 entry at 21 1/2 crosscut. The roof fall was above the anchorage. The only equipment damaged was belt structure that was located in the entry. No injuries occurred as a result of the fall. The approximate dimensions of the fall measured 50' long x 15.5' wide and over 12' high.
The linkage on the elevator doors at Pleasant Grove portal misaligned, which would not allow the elevator doors to open properly interferring with the use of the elevator for more than 30 minutes. The elevator initially broke down at 2:35 PM and was back in service at 3:30 PM
Roof fall in primary 300ft inside porthole #1 on 90N at first intersection. Pile is approximatly 20'x20'x3'to5' deep. MSHA called with in 5 minutes. There were 5 miners and 1 contractor in mine at time of incident. Everyone got out with no injuries.
Electrical power to the mine was interrupted at 4:58 A.M. There were 12 men working underground, the evacuation notice was communicated and employees assembled at the Hick's Road air shaft for removal. The electrical issue was checked out (breaker at the main substation) and employees given permission to return to work after all corrective actions were completed at 5:50 A.M.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to a regulator problem. No indviduals were on the elevator at the time. All necessary government agencies were notified according to law. The regulator brushes were seated and the car returned to service @ 2:05 PM.
An unplanned roof fall occurred in the Coalburg mine at 26 brk at 17 belt in the #2 entry near spad #510. The fall measured 30' long X 20' wide X 4' high. No injuries were reported. The area was developed on 8/6/08.
A roof fall was discovered at 5:55a.m. on the #1 unit intake 3 crosscuts outby the tail piece in the # 9 entry. There were no injuries or witnesses. Also no disruptions to the ventilation or passage of personnel.
Slope car was down due to mechanical problems.
Elevator would not operate due to a safety switch on bottom doors being off track.
The mine encountered a roof fall on the Main South Travelway, entry #3, XC #22. The fall measured 5' thick X 21' wide X 27' long. The area was initially bolted with 4' fully grouted bolts. The fall affected the haulage of supplies from the South portal. The fall area will be resupported for travel.
Unintentional roof fall at one Cross-Cut in the number two entry located on the west track.
Brush fire along the SE boundary of our property line adjacent to the freeway. The brush was ignited along the freeway & the fire traveled up the hill below the pit & to the berm along our access road. Cause of ignition is unknown. Honolulu Fire Dept responded to the fire both on & off the mine site. Fire was under control in 45 minutes & extinguished in less than 2 hours.
Rock fall occurred in 1 East Mains in the #5 entry at break #49. Fall measured approximately 20' wide x 30' long x 5' thick.
No accident - we had a unplanned rock fall - no one was affected and the ventilation was not affected - rock fall blocked travelway. This was reported immediately after getting outside
4701 Hoist would not clutch out on the right side, zero rotation relay malfunction, electricians replaced relay.
A roof fall occurred on the C6 Panel, #6 Block, #8 Intersection (Inby the Feeder). The area was bolted with 6' SuperTwist bolts and 12' PT Cable Bolts. There was one person trapped because of the fall. One piece of equipment (scoop) was involved in the fall. Please note: This incident should be reported with two (2) Accident Codes--(03) Entrapment and (08) Roof Fall.
A fire was discovered on a surface pole transformer at the mine. The fire was extinguished with dry powder fire extinguishers.

The hoist car went down at 12:21 pm due to a blown control circuit fuse. The fuse was replaced and the hoist car was put into service at 1:15 pm.

An unplanned roof fall occurred at the Alloy Powellton mine in the #6 entry, near spad #717 that measured approximately 30' long x 20' wide x 6' high. No injuries were reported. The area was developed on 3-25-10.
Sobo brake fault wouldn't reset. Tied right side Sobo control into left side UPS backup. North UPS bypassed to South UPS.
On August 17, 2010, a roof fall was discovered at crosscut #28, Main West Section, #1 entry.
The longwall gob line fell into a permanent stopping at #4 break on 8P, damaging the stopping and affecting ventilation.
A ROOF FALL WAS FOUND IN THE MAIN WEST INTAKE X-CUT 118. THE PRIMARY ESCAPEWAY WAS REROUTED AND ALL ENTRIES TO THE FALL WERE PROPED OR CRIBBED.
The mine encountered an unintentional roof fall on the MainSouth travelway, entry #3, between XC #49 and 50. The fall affected transporting supplies from the South Portal. The area was mined in March of 2000 and bolted with 4' fully grouted bolts. The area will be resupported.
An unintentional roof fall has occured on the 1st Main South Travelway #4 entry of the number 16-xc. The fall was approxiamately 30 feet in length and 10 feet high by 18 feet wide.
While examining an aircourse, the employee discovered a roof fall in the intersection of #9 Cross-cut of 23 Mains Section. This is a continuation of a previous fall in the same location. There were no persons injured, or equipment damaged as a result of this event.
A roof fall was discovered in the #11 seam in the 2 Main North between crosscut #70 and #71 in the #3 entry. The fall measured approximately 5'x35'x2'-4'. The area was bolted with 36"" fully grouted roof bolts. A clay vein in the top was determined to be the cause of the fall.
A roof fall occurred in the 2nd South Panel @ spad # 24+20, Entry #2. The fall measures 19'L X 19.8' W X 10' H.
A roof fall was discovered at 2:30 pm off of the Main North Travel Way by an examiner. The fall was located in the #7 crosscut between the #4 and #5 entries. There were no injuries or witnesses. Also no disruptions to ventilation or passage of personnel.
The hoist went down at 8:35 AM 23 men were under ground, the hotline was called at 8:53am. The men assembled at the escape shaft, the escape hoist was activated and escape hoist lowered to retrieve men. The hoist was repaired by 9:34 am and the crew returned to their work stations.
On or about 8/16/2010 an unplanned roof fall occurred on the Southwest Sub Mains, cross cut # 89, entries 3 & 4. The area was bolted with 8', 10' point anchor roof bolts with additional prop setter props. The fall was approximately 30' long x 110' wide x 15' thick. The fall in #3 will be cleaned.
A roof fall measuring approx. 16'X 25'X 4' fell in the #1 return entry near spad #2014, 1000' inby mine portal.
A roof fall occurred on #1 unit MMU 001-0 in the intersection of face area of #3 entry measuring approx. 16ft. long X 18ft wide X 7.5ft high. The area was bolted with 4ft. fully grouted bolts and 8ft. cable bolts.

An unplanned roof fall occurred in the #6 entry of the 001-0 section, this fall occurred at spad #4405, 2350' from active face. Area was developed March, 2010. Fall measures 35'L x 20'W x 5'High, area was supported by resin bolts. All entries have been breakered off. Passage of persons and ventilation was not affected. Fall will not be rehabilitated. Fed agencies notified immed.
Roof fall occurred at 59 xcut #1 entry of the 22 East Longwall section. The fall is approx. 15 ft long, by 15 ft high, by 32 ft wide. The fall affected the escapeway of the longwall on the headgate side. 5 people were affected on the face, they exited out the tailgate entry. Cause of the fall was undetected. Area was bolted 8 ft bolts, rsin assisted, metal straps & screen.
A relay on the hoist braking system failed causing the brakes to set. MSHA was notified via the hot line number. The relay was replaced and the hoist was reset.
Rodeo #2 side hoist has small kink in rope on top by bonnet. we reported to MSHA. Rodeo hoist is down at this time until repair to rope can be made.
Rock fall in crosscut 65 of north main 1 in 5 heading (20X20)
a roof fall was discovered during an inspection on a section that has not been examined since the accident on 4/5/10. the mine currently is under 103K order and was discovered by MASH, state and company officials. The fall was located just inby spad 24474 in primary escape way. Date the fall was discovered was 8/13/10
An unintentional roof fall occurred on the 3 East Mains in the No.6 heading at corsscut No.49 Spad No. 20747. The fall measured 30' long X 25' wide X 15' high. There were no injuries, no equipment was caught, and the ventilation was not disturbed. This fall will not be cleaned up.
Spring in the hoist joystick controller that would return the joystick to neutral (deadman) failed due to fatigue. At the initial time of failure, the gearing in the joystick was caused to slip, only allowing hoist travel in one direction. The spring was replaced and the joystick and hoist returned to operating condition.
An unintentional roof fall occurred at this mine on this date at approximately 1722 hrs. The fall fell due to deteriation of roof above the bolt anchorage. The fall was 20 long by 20 wide by 15' high.
The roof fall occurred in the #6 entry of the South East Panel. The roof fall is 60 feet long, 18 feet wide and 7 feet high. The roof fall will not be cleaned up and we have breakered the roof fall off.

Roof fall was bolted with Install III with resin assisted, with bacon straps and wire mesh screen on roof fall. This fell above roof bolt anchorage. Fall was caused by deterioration due to weathering.
Main breaker to the -50 level loading pocket area was tripped shutting off communication to the hoist. A bad sump pump had caused the breaker to trip.
We had a roof fall in the South Main #5 entry outby xcut #79 in entry. Fall was 12 feet long, 14 feet wide and 9 feet in height. Roof fall was bolted with Install III with resin assisted, with bacon straps and wire mesh screen on roof fall. This fell above roof bolt anchorage. Fall was caused by deterioration due to weathering.
An unplanned roof fall occurred in the #3 entry appx. 3100' outby 001 section. The fall was appx. 20'x18'x9'. No injuries or equipment was involved in the roof fall.
Rock fall occurred on 1 West Mains at break 51 between #4 and #5 entry to the right of spad #573. Fall measured approximately 18' wide x 30' long x 5' thick.
On 8-12-10 at 3:24 PM, Mine Foreman found rock fall on the highline at break #217 on 4 South Mains. This was in the Number 5 Entry. MSHA was notified at 3:30 PM. Permission to re-route highline and lifeline was given. This fall will not be cleaned up. Area was dangered off. Timbers and cribs were set. No disruptions in ventilation were encountered. No injuries occurred.
On 8-12-2010 a rock fall was found on the belt line in the #4 heading. The fall was approximately 20' wide X 70' long X 6 to 8' high. This fall was in our secondary escapeway.
A roof fall occurred on the mainline track at Survey Station 149+50. The fall is approx. 20' long and 30' wide, including the intersection.
Upon slope exploration of this in-active mine a roof fall was found at 1220' mark in the slope. Approx. size of fall is 15' high, 19' wide and length is unknown at this time. Cause of this fall was deterioration due to being flooded with water for many years. This report is being requested by MSHA on 3-2-11.
A roof fall appx. 240'x19'x6' occurred in the #7 entry on 17B. No equipment or persons were involved.
Material fell from the rib on the level 1 to level 2 decline. The material that fell damaged the conveyor disruping regular mining for more than 1 hour. No injuries or illnessess occurd as result of this accident. There was no immediate danger as no one was in the mine when the event occured. MSHA was notified.
A roof fall was discovered at 10:40am on the main north. The fall was located at x-cut 23 between #5 & #6 entry at spad 16 & 32. The fall was 50'L X 18'W X 8'N. The were no injuries (or witness), or disruptions to ventilation or passage of personnel.
The MVR process startup resulted in an undervoltage condition that resulted in power being lost to the #2 hoist and reset would not function. The mine was evacuated at 12:22PM and completed at 12:28PM. Power was restored to the hoist at 1:20PM. The hoist was tested and miners returned underground at 1:20PM.
A supervisor found a roof fall in the # 6 intake entry of the Mill Fork Mains. This entry is not a normally traveled entry. The roof fall was approximately 20 feet wide, 60 feet long, and 5 foot in height. The roof fall was immediately reported to MSHA and blocked off to prevent entry. Current plans are to re-bolt the area.
The mine roof in the right return ( no. 4 entry) of No. 4 section (mmu 004-0) fell above the anchorage of the roof bolts, beginning at spad 27540, and extending inby for approximately 240 feet with a width of approximately 20 feet.
Roof fall was found in the C panel off Holt Road submain, #5 entry, spad 9+10. The fall was appr 30 ft by 20 ft by 6 ft plus. The fall is appr. 3200 ft from active workings.
On 8/11/2010, an employee at the Somerset Portal of Mine 84 tried to use the elevator at this facility. He found that the elevator doors were stuck in the closed position. The elevator repair company was called and placed the elevator back in service at 6:00AM. There were no injuries, or equipment damage resulting from this event.
When longwall tailgate shields were lowered for advancement, roof rock fell from above the shields, resulting in a tailgate blockage. Blockage occurred in entry 3, inby crosscut 53, E1 Headgate.
The Crabapple elevator was not operational for a period of more than (30) thirty minutes. No injuries reported as a result of this incident.
A roof fall occurred in the West Mains #5 Entry at station number 1+78.01. The roof fall was above anchorage. There was no known damage to equipment and no injuries. The approximate dimensions of the fall measured 45' long, 15.5' wide and 12' tall. The fall was discovered by mine examiner at 8:49 AM.

The fall did not affect persons, ventilation nor equipment. The area has been dangered, cribbed and supplemental roof support installed.
An unintentional roof fall has occured at the 43xc of the 2nd Main East extending to the 44xc in the #3 entry. The fall was approximately 60 feet in length and approximately 7 feet high and 16 feet wide. The fall was timbered and dangered out.

A roof fall was discovered at 10:40 a.m. on 08/10/10 by a fireboss. Location Westmains 49 BK to 4 CC. The fall was 14' wide, 32' long and 6' high. The fall will not be cleaned up. Both entrances to the fall have been cribbed off.
Mine had an unintentional roof fall on th e#9 belt line at break #41 & # 42. Fall was 30'L X 18'W X 5' H.
Contractor was filling oil tank on the 2nd floor of 6RR when the tank overflowed and spilled on #5 calciner causing the oil to ignite the fire.
A water discharge pipe broke in the Pace X-over main return aircourse, adjacent to a return entry sump. The broken line was not discovered for several hours, as such the water in the sump overflowed to a point where it blocked off a return aircourse. The mine was under a previous K Order, mining was not taking place. Ventilation changes were minimal.
A ROCK FALL WAS FOUND IN THE NUMBER FOUR (4) ENTRY OF THE 3-WEST SUB-MAINS ON 8-9-2010, THE APPROXIMATE SIZE OF THE FALL WAS 20' WIDE, 20' LONG AND 10' HIGH. THE ROOF WAS A LAMINATED STACK ROCK, AND HAD BEEN SUPPORTED WITH 6' TORQUE TENSION AND 10' ROPE BOLTS.
At the start of the graveyard shift at approx. 10:40 p.m. the 19 member crew loaded onto the mancage. As the mancage was being lowered the pinion emergency brake lost hydraulic oil pressure and the brake set. The mancage and crew were trapped in the shaft until 11:25 p.m. when the emergency brake was released and the crew was brought back to the surface.
Skip door wouldn't close and skip was not allowed to leave the headframe. The gate mechanism was cleaned and a build-up of material was removed. The gate closed and the system was retuned to normal operations.
A roof fall was found at 3:45 on 8/9/10 at xc 106 entry 4 main south travelway. Fall was in four way intersection that had been supported with 4-8 full resin bolts. No property damage or injury occurred. Fall was in a secondary escapeway, an alternate route was marked for travel. There are no plans to clean this fall.
At an unknown date an unplanned roof fall occured at cross cut 181, #2 entry, in the 4th Main West. The fall was approximately 18' wide x 20' long x 18' high. The area was bolted with 6' point anchor roof bolts. The fall has been cleaned.

Ee had just dumped a load of concrete stone; he thought the bed was down and he took off. The bed was up and he hit the cam span. An alarm is to sound when the truck is in gear and the bed is up. The alarm did not sound.
Rock fall occurred on 1st right off 5 West # 2 entry, 5 pade # 9939 inby 5 pade 1 Brk.

The Cedar Creek Elevator was out of service from 12:30 pm until 3:00 pm due to replacing the governor's rope.
A roof fall occurred at the bleeder, next to the last open crosscut between #2 and #3 entry. The fall measured 18 feet wide, 50 feet long & 6 feet thick. Persons, equipment nor ventilation were effected. This area will not be rehabilitated.

The man hoist at New Era Mine stopped functioning at 3:40 P.M. due to an electrical malfunction and was back in operation at 5:00 P.M. when the test run requirement was completed.
At 4:25 p.m. an abandoned non producing oil well casing was hit on the # 1 unit in the #10 entry. A hole was made in the casing by a miner bit. A 300' barrier was left around the oil well location. The well was drilled in 1959 and location was spotted off of a state of KY. Oil and gas map.

#31 Hoist down- bad connection on limit switch in head frame. Hoist was down for 4 hours 10 minutes.
The weekly examiner discovered a roof fall located between XC 24 and 25 in the return entry of A-West. The area had supplemental roof support installed due to the faulted area. There was no weight on cribs, timbers or bolts. The entry had been traveled within the last week for EO1 inspection. The roof fall closed off the escapeway that would be used by the Main West section.
4701 #1 Hoist went down with electrical problems, a bridge rectifier was found to have malfunctioned. Electricians found the AC to DC coil that the rectifier charges had malfunctioned which caused the rectifier to fail. Electricians repaired hoist.

IGNITION: Employee was burning off of the face side crusher on No. 2 LW after wetting down the area. A methane test was taken prior to and continuously during burning and had 0.0CH4. He had burned one bolt off and starting to burn the second one when a flame appeared on the Mine Floor. It burned less than 5 seconds and was put out with water. No injuries were incurred.
At approx 5:20 pm a 40 ton articulating truck was traveling along a haulage road impacting a 63"" high berm. The truck traveled across the berm and fell 72 feet to the mine floor below. The victim was extracted from the truck by EMS and airlifted to the Hospital.
EE was loading boat onto trailer, another EE in pickup started to pull up, wheels started spinning. Other EE went to get loader and chain, when he got back EE told him that boat had floated off trailer, EE was standing beside pickup. The other EE went to shop to get hook to try to retrieve boat, when he returned, EE's boots were by the water and EE was missing.

Employee was removing a metal sign from a post when sign dropped down on employee's 5th phalange on left hand. Employee lost the tip of the finger. Employee was not wearing gloves at the time of the incident. **Employee had little finger amputated at the middle distal.**
Loading a shot, uneven ground, hard rut, twisted left knee & fell down.
Conveyor was not turning during crusher start-up, investigated to make sure it was free. Start button was in off position, breaker was trapped, it was reset and belt started even though it was off.

Miner was shoveling at the tailpiece when the spillboard fell and hit him. Employee suffered broken toe, mashed (not broken) hand and strained back.
He said he was looking up at roof bolt, twisted neck.
EE was working putting in a differential, after lifting the differential and lining it up, the nut wielded to the differential to stabilize the clevis assembly broke free causing the differential to fall hitting the corner of the pallet below it in return causing it to roll striking ee above the Rt knee. X-rays show no broken bones or limbs. Release the next day.
New 4"" hose on compressor blew off fitting when pressure was being brought up and hose whipped employee. Left arm contusion, left foot contusion, broken nose and bruised right knee and elbow.
Miner was shoveling at the tailpiece when the spillboard fell and hit him on the head and shoulder. All tests (x-rays, etc.) were clear. Ended with bruises and being very stiff. Family doctor gave him a week off.

EE was replacing a bucket jack on a scoop. He noticed a flag pin had become fouled behind scoop ram blade. He tried to remove bent pin when a splintered piece of metal penetrated his right thumb.
On 8/30/10 employee was greasing the bottom right pin of a 988G when the operator tilted the bucket back rolling it onto the right part of the right foot smashing his foot. This crushed the two right bones going to the two right toes just behind his toes.
Was pulling a tail piece with a scoop when the chain broke and came back and hit him on the left shoulder.

Employee had been injured while working around the house on days off, condition was aggravated by work. Symptoms include numbness and tingling to left lower extremity. Symptoms increase while sitting for short periods of time. Employee referred to his personal physician for evaluation.

Employee was replacing a hydraulic fitting when his foot slipped injuring his right knee. Employee later required treatment and started missing on 01/06/14.
He was trimming a piece of rock when a small chip of steel from the hammer hit him in the right side of the neck causing a puncture wound.

Member was in the process of removing a rock measuring 2'Long X 18""wide X 8""thick from inbetween the cats of the miner and the pan, when he bent over to remove the rock he felt a pain in his lower back.
EE was packaging 50# bags. He felt a tightness in his back. We had a Save a Back class on 8/11/10. EE decided to do some exercises that he had learned in the class. At that time he got a sharp pain in his right side/lower back. He was unable to finish the day. He has a doctor's appointment in one week.
Headman was lifting an 8' curly beam in the belt entry. He slipped on a piece of belt that was covered with mud causing him to strain his lower back. EE continued to work until 9/11/10 at which time he visited health plus in Kanawha city and was put off work.
Operator was getting loaded at the trackhoe when a rock was dropped into the bed of his truck causing the employee to be shaken and jarred around inside the truck.
While picking up a bag of rock dust to fill duster, employee pulled a muscle in his groin area.
The employee was using a track jack to put a car back on track. When lowering the jack manually, the top of the jack free fell pinching his left middle finger causing a fracture.
Employee was weighing a sample and bent over to pick it up off the scale and felt lower back pain.
EE was taking drill steel apart when his right ring finger was mashed between 2 pieces of drill steel.
EE was using a pry bar to dislodge clay and rock from crusher slide. The material broke off, and caused the pry bar to kick up and hit him in the face.
EE was setting up tooling to press out the king pin on the Rock Truck and while hooking up the chocker cable to re-position the 100 ton ram sitting on the spindle the ram fell over and the cable hooked on his left thumb. First aid was adm. and EE was transported to hospital for treatment. EE was referred to a orthopedist by the ER Dr.
The employee said that he may have hurt his knee getting off a forklift, but he wasn't for sure. There is pain and swelling around his knee.
EE was hooking chains to a cab to unload it off a step deck trailer. EE forgot it was a stepdeck assumed it was flat and fell. EE landed on his back and then rolled off the lower portion to his head.
Ee is a repairman. He was using a buggy for transportation when he hit a timber with the buggy. This moved the timber so that a piece of rock fell from the top and hit him on the neck and shoulder.

EE was walking on catwalk of drill slipped and turned his right ankle.
EE was welding on a 992 bucket and at the end of his shift was picking up his tools and scrap metal and placing it in the back of his mechanics truck, he hit his knee on the bumper and bruised his knee cap. Lost 1 day of work.
Member was walking down the slope belt on the track side, when he slid in some mud and rock dust. Member grabbed a hold of the slope tail piece to stop himself from sliding when he felt a pain in his left shoulder.
Employee was watching the belt & had his hand on the belt rope & a small piece of draw rock fell & mashed the end of his ring finger on left hand. Tip of left finger was amputated by doctor.
Injured was tramming roof bolter when piece of rock fell striking right middle finger.
Member was preparing to load top structure into a scoop bucket when he bent down to pick up the structure he heard and felt a pop in his lower back.

Employee was removing a rock from the travel road of her shuttle car. She stated, she strained her back while moving the rock. The incident became reportable on 9/8/10 when she was taken off work.
Employee was walking through the Hydrate Baghouse at the end of his shift and was stung by a bee near his right eye.
The man was tramming a personnel carrier when the steering wheel spun around and the steering wheel knob struck the man on the hand.
Walking up the hill in mine trying to step over shuttle car cable, fell after feet came out from under him falling on right side twisting right knee.
EE was on his knees cleanig K-4 switch with a hammer, when EE started to stand, he felt pain in the lower right back. EE continued to work until 10/26/10 when he went off work due to back pain.
While hauling & piling topsoil; reclaiming, with TS 24 Pan, rear slipped and began to slowly roll over. Pan rolled completly over once with operator buckled in. Pan then set upright long enough for operator to unbuckle and dismount. Pan continued to roll down slope. Operator landed on right shoulder & right side ribs.
Employee was hitting on a track pin when a piece of metal hit him under the chin. Medical treatment was provided to remove the metal.
Employee was hand scaling loose material in the mine roof when he felt a pop in his right shoulder and the shoulder started to ache. Employee did not start losing time from work until Tuesday, November 16, 2010.

Injured employee was walking between the rib and a shuttle car, when a piece of draw rock fell from between the rib and first bolt and struck him on the back of his neck, just below his hard hat. Rock measured approx. 3'x2'x1""-2"". The ER physician excused the injured for 7 days due to a strain of the neck and tissue injuries.
EE was walking through a door in the shop and tripped on a fan cord, stumbled and broke a bone in his ankle.
Member was walking by scoop and tripped and fell hitting his head on the scoop canopy.
Repairing a knock-out in an electrical box. Facial, neck and arm burns.
Employee was assisting two other employees with the removal of a cleanout door on the discharge side of a 10"" pump, in an attempt to unplug the line. They were not aware that there was a buildup of hot water and steam in the pipe behind the door. As he was removing the bolts, the door broke free and the sudden escape of steam and water burned the employee on the left forearm.
Injured employee was loading rail ties onto a flat car, when he smashed his right hand between the tie and metal portal door, causing pain and a laceration. X-Rays revealed a fracture of his right little finger and he received 4 sutures in that finger. The physician has excused the injured from work until 10/25/10.
Lifting bucket up on low trac when one panel of the K-stopping slid off top striking left wrist.
While permorming maintenance work on belt press #2, the employee reached out to place a come-along around a pulley and strained his back.
Employee was operating haul truck, hauling material to the primary crusher. Truck traveled through berm on haul road causing multiple injuries. MSHA was notified immediately. This accident is under investigation.
On 23 Aug 2010, at approximately 1:30 AM, individual was loading AN on bulk truck from AN storage towers when he felt a sharp pain in his abdomen from straining to open AN tower gate. Individual was diagnosed with a hernia on 23 Aug. and received surgery to repair the damage.
Employee was standing on rock while shooting elv. Rock rolled out from under him causing employee to fall on to left leg.
Employee claims to have had an accident on 8-23-10 but did not report incident to the mine until 9-3-10. At that time the employee was sent to the panel of Doctors. On 9-6-10 the employee took vacation and company could not talk to him until 9-13-10 to report the accident.
He was assisting other EE's while installing ramps on an overcast, the ramp got stuck and he pulled on the ramp, felt pain in right side. He continued to work at his regular duties until 9/1/10, when he was removed from work for surgery to repair inguinal hernia on the right side.
EE was exiting the A.L. Lee Lo-Trac underground forklift 2 breaks outby #4C Head drive in the #3 entry. He placed his hand on top of the canopy to assist himself out of the deck and a piece of draw fell and struck him on his left thumb, causing a laceration. He was taken outside and transported to the ER where he received stitches to close the wound.
Miner operator was attempting to remove a rock on the opposite side of miner (right side), miner was shutdown and a rock fell out between the bolt approx 4 ft x 4 ft 5 in thick striking him on the head pushing him to the ground. He was boarded and collared and brought outside to the ambulance who decided to fly him to UPMC Presby in Pittsburgh for precautionary measures.
While working on a scissor deck, employee was working in articulation and the machine articulated to the left pinching employee between cab and deck. Was taken to NNRH where it was found he had fractured ribs and a punctured lung.
Operator/Fueler was attempting to put antifreeze into equipment, when he missed the opening for the radiator and the antifreeze splashed on him.
EE was operating a haul back truck. It hit a rough spot in the road which jerked him sideways in the cab. This caused a strained back. Two days off and physical therapy has allowed EE to return to work with no problems.
EE was helping re-install inside drill boom arm. When the arm fell it struck the EE on his right foot.
On 8/22/10, shift foreman was walking through the raw coal tunnel and stepped on a rock on the floor and twisted his knee and struck the belt structure. EE completed his shift and later sought medical treatment. EE had surgery on 12/19/10 and his 1st day away from week was 11/25/10.
Employee was taking a pipe apart above clean coal screen on 3rd Floor. Twisted left knee stepping down off screen. Knee swelled up over night and he couldn't put much weight on it the next day.
While using a pry bar to re-install the 1/2"" cable on the dredge winch the pry bar slipped causing the EE to hit his left leg on the deck.
EE was sitting in 54 portal bus at the ""hole"" switch. 58 bus moved from the spur to the mouth and didn't see 54 bus sitting there. 58 bus struck 54 bus causing EE to receive whiplash.
Employee was cleaning ditches in the plant basement with a shovel. He lost his balance and fell to his left knee. He felt pain in his lower back and left knee.
Two EE's were attempting to clear a plugged sample point piping, which is a routine duty. As they attempted to clear the pluggage, the sample point piping assembly sheared off from the process pipe it was attached to causing caustic solution to spray both EE's. EE's sustained 2nd and 3rd degree chemical burns.
While performing maintenance, employee stepped down off of the tail frame of the secondary cone supply conveyor. Employee lost his grip on the handrail and fell approximatly 30"" on to the edge of a concrete block, fracturing his left hip.
Dumping a wheel barrel of clinker from the reclaim tunner, EE started to feel spasm in right shoulder area.
EE instructed the scoop operator to pull back the stinger on the scoop. They were pulling a roll of belt into the bucket, as the stinger retracted, EE's hand was caught between the frame and stinger causing a fracture and soft tissue injury to his left hand.
Employee was assisting with drilling for roof bolts and drill toppled striking employee in the neck.
The employee drove the loader into the shop for maintenance and to take lunch break. As the employee was stepping down from the cab of the loader, he lost his grip and fell to the ground injuring his arm.
Employee stopped dozer to look behind when his neck popped. He could not turn head to side afterward. The mine believes due to no noted mechanism of injury that this is not work related. Potential causal factors found include employee having a metal plate in his neck from a previous non-work related event, and that he was ""breaking a philly"" the previous day.
Employee was lifting a fender for a shuttle car with help form another employee, when he felt a sharp pain in his right lower abdomen. Through an investigation on 9/8/2010, it was determined that this was a work related injury and in fact did occur at the mines.
While cutting an access door for a water line in the skirting of a mobile home. He was setting up the saw blade broke, kicking the blade back into his leg just below the knee.
Employee was hit in the leg by a 16 inch pipe that was being moved by a loader.
EE had moved the continuous miner to the #5 entry on #2 unit. As he was setting up the machine and dropping off the trailing cable, it appears that he came in contact with the miner tail. It is unclear to him how the accident happened and there were no witnesses.
Employee was operating the long wall shear, when a rock fell between the shields, hitting the employee on the neck area.
Employee was standing behind a mill block that was 10' x 6' x 3'. Loader operator set another block down in front of the block employee was standing behind. When backing out, the second block tipped into block by employee, causing it to tip into employee, pinning him under the block. Caused crushing injury to lower right leg, between knee & ankle. Also possible pelvis fractures.
Employee was walking from a dozer to a hauler and tripped over a small berm created by the dozer pushing, he fell to the ground. He caught his fall with his right arm and stoved it up. He continued to work and started missing work on 9-7-2010 due to complications.
WHILE ASSISTING VACUUM TRUCK OPERATIONS EMPLOYEE WAS BITTEN BY AN INSECT, TYPE OF WHICH UNKNOWN AT TIME, DETERMINED UPON TREATMENT TO BE A BLACK WIDOW SPIDER.
Employee stated that he had been installing mining timbers - when returning to equipment, he slipped on mat causing him to fall, striking rt. knee.
EE was on the drivers side of the truck to unlock hub when another EE backed over him putting him approx. under back end of the truck between the wheel and the rear end. Shift was over at 5:00 p.m.
Employee strained his right shoulder while steering an R90 Euclid haul truck
EE was working at a seal site in B- bleeders and he had gunite/concrete in his eyes.
Employee was unloading cribbing material from scoop bucket when he felt pain in right shoulder. Received medical report 9/1/10.
Employee was drilling a lifter with a jack leg drill. He tried to pull the steel out, and it would not come. So he tried again, this time he squatted down to use his legs and he felt a small pop followed by a burning feeling in his left groin area.
He was assisting the center bolter while installing 16' bolts. He was retrieving the drill steels, when the bolter operator dropped the drill head it caught his left foot under the drill head, causing a contusion to the left foot.
Employee was sliding a large rock off of a dozer arm. When the rock began to fall he twisted his body to get out of the way. He experienced pain in his left side middle back.
WHILE MINNING COAL AT THE FACE A PIECE OF COAL FELL BETWEEN THE PINS CRUSHING HIS LEFT FOOT.
EE was welding legs for skirt rubber on the tunnel belt he was tied off with a safety harness on the side where there was no catwalk to stand on, it had started to rain and he lost his balance, his safety harness prevented him from falling. He has continued to work since 08/18/2010 but had problems with his right shoulder and he had surgery on 1-27-11 to repair his rotator cuff.
Operator of the lube truck stepped out and onto a rock, twisting his ankle. Intially the operator felt the ankle was ok and continued to work. He was later seen by a doctor and restricted from working on August 19th due to a severe sprain.
Employee was walking and struck his head on a roof bolt. Employee received slight neck pain.
Miner was lifting a tailpiece by himself and pulled a muscle in his lower stomach.
EE stated he was walking between two cribs carrying a cap piece and tripped on a rock in his walkway. He twisted his right ankle and knee. On his way to the mine floor, he struck his left shoulder on a crib.
Pinched and cut left ring finger with sharp piece of metal.
Employee was operating a roof bolter when he was struck on the shoulder and arm by a piece of roof rock. The piece of rock measured 18 inches by 12 inches by 1-4 inches. The result was a laceration that required 8 stitches.
EE was closing a set of supply doors and the doors slammed shut on him causing contusions to his back and ribs.
EE was dismounting a Mack dump truck at the end of is shift, as he was stepping down, he said his knee popped and could not walk. Steps were clean of debris and in good condition. He said his knee just popped and nothing with the vehicle or the ground condition had anything to do with it. He stated it was the same knee that had been swollen a few weeks ago. Taken to ER, has sprain.
Employee was working underground when something on the equipment arched, it ignited some hydraulic fluid. He used fire extinguisher & it was put out. He was sent to medic for evaluation due to fire extinguisher dry chemical & smoke inhalation. On his next shift, he reported to work on 8/18/10 & worked 1/2 his shift, and he said he had a doctor appt, and restrictions were given.
While picking up a bucket of refuge EE was curling his bucket in the pile and the rear wheels came off of the ground and then came down hard causing the seat to bottom out. EE states that he felt pain in his lower back.
EE was raising belt structure at 20 block on 1-South belt. He was using a ""bumper"" jack. While doing so, the jack slipped out allowing structure to drop. He had his right index finger pinched between a belt chain and belt hanger, while adjusting the slack.
Rock truck driver left work at 11:15 AM to go see his doctor. He said he wasn't feeling well and he felt his face going numb. He mentioned that the bed on the RT23 truck was coming down hard, but didn't think this had anything to do with the way he was feeling. ***Days later he said the Dr said the bed coming down hard could have been a factor.***
Employee was welding on the wash rack. After he unhooked his safety harness to reposition himself, he slipped on some gravel and fell 16 feet to the ground below. There were no witnesses to the incident, he called on his cell phone to report the fall to a co-worker. He bumped his left temple and had a tail bone contusion.
Employee was hand trimming & packing stone at #3 saw conveyor at thin stone bldg. Employee experienced pain in right elbow running down to middle two fingers. Injury was originally reported on 8/17/10 & employee continued to work until 2/28/11. At this time due to current Dr's restrictions, employee is not working and is awaiting elbow surgery scheduled on 3/17/11.
Employee was removing a tooling bolt with a hammer and wrench. When he hit the wrench with the hammer, the wrench bounced back , catching his finger between the hammer and wrench.
The EE slipped while climbing down the dozer steps bumping his head & right leg on the dozer ladder, resulting in a bruise, received medical attention as a precaution.
EE was backing himself out of the cab of IV with 3 point of contact when he planted his foot incorrectly and sprained his ankle. EE originally signed Refusal of Medical Treatment Form. EE was seen at a medical clinic on 8/17/10.
Employee was cutting skirt rubber when the blade slipped and cut his finger
Employee stated he had lifted up the Cont. Miner trailing cable to get it over against the rib out of the roadway more. As he was walking the cable over he stumbled on some loose gob and fell toward the rib. He stuck his arm out to catch himself from falling into the rib and felt sharp pain in his shoulder.
Removing fitting using a crescent wrench. Wrench broke while he was pushing and caused him to fall and twist his right ankle. Fractured ankle.
EE stated that he was standing behind rockduster when the #2 mantrip came in behind and caught his right ankle between the mantrip and the rockduster.
While operating overhead Gantry Crane controls with right hand, employee guided and latched hook into the completed catwalk with his left hand. Upon pressing the up button to raise the catwalk, the hook twisted, crushing his left thumb between the hook and the catwalk causing laceration.
Individual was assisting in moving miner cable from outby rib to inby rib. He said when he pushed on the cable he felt something pulling in his back.
EE was welding on the deck of a barge on 8/16/10. He reached around his body for a welding rod and when he turned he felt back pain. He went to the doctor on 8/16/10 and was diagnosed with back strain. He was put off work for 5 days, returning to full duty on 8/24/2010. This incident is being report August 26, 2010 which is within 10 days of seeking medical attention.
Was hanging up mier cable when he felt a sharp pain between his shoulder blades.
Employee was climbing onto his D11 dozer and slipped and fell spraining his left hand.
Employee was dumping a load of rock when the truck started to turn over, he tried to let the truck bed down but the truck turned over. The truck flipped four times before coming to a rest on all 6 wheels.
Employee was tightening the radio antenna, when she received a shock.
Employee was changing bit inserts in the C/M head using a joy insert utter. He was using a chisel as a shim behind the puller. While doing so the chisel slipped out striking him on the left hand causing a laceration.
Drilling hole for resin bolt, EE smashed his middle finger and nail.
The injured was helping move a roofbolter tire in the supply hole on 14 Right section. They were trying to lift the tire to get some strips of belt form underneath, when he felt pain in his lower back and radiating up his left side.
Employee was inspecting bull gear and pinion when he lost his balance and smashed his left hand in between the dryer tire and the trunnion.
Stepping off a water truck, missed step, fell to the ground. Seen at ER, diagnosed with head injury, released to full duty. Contacted safety on 9/3/10, was having pain in head and left shoulder. Seen 9/17/10,diagnosed with a shoulder strain, released to full duty and referred to physical therapy. He is not progressing. Medical reportable for treatment due to physical therapy.
Employee was installing roof bolts in the #7 entry of unit #4. As he was raising the boom, he lost his balance. He placed his left hand on the boom to steady himself and continued the upward movement with the boom. His ring finger of his left hand was caught between a bolt head used to adjust the canopy post and the boom, slicing the fingernail off the top of the finger.
EE was hanging curtain. He turned and ran into scoop canopy. He said it knocked him back and he twisted his shoulder. He did not report this until late on 8/16/2010. There were no witnesses.
Operator was exiting haul truck and when she reached the bottom of the ladder she turned around on the platform to exit the bottom portion of the steps and she fell down the steps to the ground and hit and cut her right knee.
Injured was helping to change belt structure. Injured fell against rib and structure brusing his ribs.
Laying on top of belt ratcheting chain hoist and felt a pop in his left rib area.
Employee stepped through a bar screen when walking across it.
The move crew foreman wanted to move shuttle car out of way to make belt move. He pulled 15' cable off reel because pump didn't start. He told another EE to start car and everyone was clear. Reel began to take up and cable hit foreman, knocking him down. He said his right leg and lower back felt numb. X-rays were negative. He remains off work.
Employee was walking and slipped on the shop floor, he ceased work and went to Hospital
Member was rock dusting offside of belt line, when his knee gave out and he fell onto the beltline that was around 33"" off of the mine floor. Member then rolled off of belt line.
Lotrac operator was going through scoop doors at the end of the track. Lotrack operator saw employee standing next to the door and when the operator went through the door he heard employee make a grunting noise. He then looked around and saw employee on top of the lotrac bent over. The lotrac operator had to back lotrac up so that the employee could get off the lotrac.
Sulfur ball fell out and bounced off drum, striking injured in the wrist and leg while setting bits.
The employees was using a bench saw to cut plycem. at the end of the road he turn off the machine but pass his left hand near the blades and touching it and he got injuried
Employee was bitten by a copperhead while retrieving a hose from the hose trailer.
Got off Shuttle Car to check cable. Tripped on cable and fell down, fracturing/breaking left wrist.
EE was helping carry an injured EE out of the mine. The stretcher shifted, causing him to carry most of the load. He felt pain in his lower back. EE did not miss work until 8/17/10.
Employee was dragging cap pieces and wedges through a low area 48"". While doing so he struck his head on a one hole board causing pain in his head.
Employee was handling cables, lost his balance, right foot stuck in mud causing him to strain right knee. Started missing work on 8/16/2010.
Employee was walking in the 14H section and rolled his ankle, which put added pressure on the inside of his foot. Employee experienced a fracture to the 5th metatarsal on his left foot.
Employee was harvesting cu cathode using pry bar. Bar slipped and hit employee in left knee, causing a contusion.
Employee was riding mantrip on the way to section when the mantrip hit a bump in the travelway. Employee felt pain in lower back.(Resulted in strained Lower Back)
Installing attachments to Hi-Wall Miner. Employee had finger between Miner and attachment. Cut tip of finger #4 (ring finger).
Wet foggy conditions causing material to become heavy, shoveling material away from equipment after build-up, strained rib/abdoman area.
Walking to help pressure wash, and twisted right knee.
The EE alleged that while stepping down from equipment he slipped on the steps falling to the ground.
SLIPPED ON WET CONCRETE, DID NOT BOTHER HIM WHILE AT WORK. HE WENT ON VACATION, RETURNED TO WORK, IT WAS BOTHERING HIM SLIGHTLY BUT HE IGNORED IT. FINALLY ON 9-1 HE REPORTED IT TO MANAGEMENT. WE SENT HIM TO A DOCTOR. DOCTOR DID XRAY DID NOT FIND ANYTHING. HE CONTINUED TO WORK. DOCTOR SENT FOR MRI. MRI DETERMINED TORN LIGAMENT REQUIRING SURGERY.
A supply locomotive struck a track jeep on 8Q track with three employees sitting on the jeep. One of the employees suffered a contusion to the left elbow.
Injured was operating scoop and crossed track dropping into hole causing the injured to strain back and neck.
EE slipped on a rock and might have twisted his back.
Pinched left little finger between a cradle roller and the belt while closing a water valve.
Employee stated that he was cranking the handle on the leveling jack on the utility trailer, so as to unhook it from the dump truck, when he felt pain in his lower back area. He went to doctor on 8-13-2010 and incurred lost workdays starting with that date.
Walking down belt line, ran side of head into broken piece of mine screen, cutting head. Cause of injury was lack of concentration. Mining equipment/system was not a factor, ppe was not a factor, compliance rules/regs was not a factor, miner profiency is a possible factor.
EE was operating a 38 ton motor. While turning the pole he was attempting to replace the harp on the wire. The harp missed the wire, he released the pneumatic controller and his arm was pulled up making contact with the DC trolley wire.
EMPLOYEE STATES SHE WAS LOADING AIR PUMP ONTO BACK OF TRUCK WHEN SHE LOST HER FOOTING. SHE FELL TO THE GROUND AND THE AIR PUMP LANDED ON TOP OF HER RIGHT LITTLE FINGER.
EE was changing out a pump when the shaft slipped and caught his two left fingers between the bottom of the sleeve nut and top of the gland. The ee recieved stitches on one finger and further treatment on the other finger. The ee has not returned back to work.
The member was raising the hood of a 6370 Hatachi Excavator to check the fluids as part of the pre-shift inspection. While attempting to raise the hood, the member lost control of the hood and it immediately started back down. The unexpected change in direction placed strain on his lower back.
Employee was helping to change bits on continuous miner. Pulled a rock off of the head striking himself on top of the left foot. Diagnosed with fracture to the 5th metatarsal and 5th toe of the left foot.
EE stepped off rail motor and twisted his right knee. Started missing work 8-13-2010.
ANKLE - SPRAIN - EMPLOYEE WAS GETTING OUT OF HIS WORK VAN, STEPPED ON A LARGE STONE AND TURNED HIS ANKLE OVER.
Employee was removing the water pump from a D11 dozer and the pump slipped out of his hand, catching his hand between the water pump and frame.
EE was putting cribs under cont. miner when he stood up he felt his left knee pop.
Employee ""ALLEDGELY"" strained his back while lifting roof support pizza pans.
Employee was pulling on lever to let prell in the prell truck and his back started to hurt.
EE slipped and fell resulting in a fractured bone in his right ankle.
Employee was walking to the power center in the #5 entry @9-1/2 xcut south to turn power on to shuttle car when he stepped on a ledge/hole and twisted his ankle.
Employee was recovering chain used to hang ventilation tubing from a roof strap when a rock fell from between roof straps striking employee on top of the left foot. Employee was diagnosed with fracture and ligament damage to the left foot.
Employee was operating a 100 ton rock truck. After dumping a load, the bed came down and jarred the truck, causing the employee to be shook and injuring his neck.
The miner was working in the Charlie south raise he was lifting a large timber 8"" X 8"" X 10' and pulled the muscles and tendons around his left elbow. The accident was not reported until 8/19/10. The employee said that his arm hurting, a lot of pain and needed to go to hospital to have x-ray.
Employee reports low back pain from entering/exiting loader. Chiropractic care sought on 08/11/2010. Anticipated return date of 08/16/2010.
Employee was operating a lathe completing what appeared to be the final stage of machining 3/4"" round steel rod. Employee was using a piece of emery cloth polishing the steel rod, it appears that the emery cloth either caught on the steel or wrapped over its self and pulled employee's left hand into the lathe while it was running
EE was in the process of removing a sprocket with another mechanic. EE stepped back to observe the sprocket while the other EE struck it with a 8 lb sledgehammer. A piece of metal came off and struck EE in his right forearm. First aid was given and EE was transported to ER. Dr. referred EE to a surgeon.
EE was operating a Cat 988F end loader. Another end loader came around small pile of coal and struck the rear of the other loader.
Employee was walking around the standard cone to the first van when the doors on the VSI 2100 came open and started throwing rock out. A rock flew out towards the employee and hit him in the mouth.
Miner is a miner operator/miner operator helper. A small piece of draw rock fell from the top and him him on the lower left leg and foot. No bones were broken. He was off three days and came back to work no problems.
Foot slipped on step and EE fell to the ground and his foot twisted.
The area was muddy, with plank in the bottom. The miner was carrying crib block, slipped on plank twisting left knee.
Employee was cleaning out a clogged chute with a water hose when material dislodged from chute wall striking him in the hard hat which resulted in his face impacting the chute access door.
EE was carrying a fire extinguisher in muddy conditions to replace one that had been used and slipped in mud and when he caught himself to keep from falling, he strained his lower back.

Employee was on Secondary Crusher belt when a co-worker started up the plant without following procedures. A second co-worker noticed that the plant had been started up with the employee on the belt and immediately yelled to stop the plant. The employee suffered minor injuries, which resulted in time off from work.

Wet foggy conditions causing material to be heavy and hard to handle. Shoveling the build-up from the belts, heavy material caused a wrist sprain/strain.
Employee was lifting a cylinder out of the truck without sufficient help. The cylinder was greasy causing it to slip from the employees grip. When the cylinder slipped, it lacerated and smashed the employees right pinky finger.
Employee was removing a metal plate from a magnet when it retracted and pinched his finger cutting the tip off.
Employee was prying rock with pry bar and bar slipped off striking employee in head. Employee was wearing hardhat....
Employee stated that something hit him as he was getting in the shuttle car. An investigation was done and management believes that the employee slipped and hit shuttle car. Section was wet and muddy.
Employee injured his left shoulder while beating on ore bin on Aug 9, 2010. Employee went off on MLDA for shoulder surgery on 1/19/11.
Employee was installing his second section of drill steel in the drill pot when a piece of rock fell from the mine roof. The rock struck his right hand & smashed his right middle finger between the rock and the drill pot of the roof bolter.
Employee encountered smoke coming from face drive. When he went to check it out, he got into the smoke and it caused him to have difficulty breathing.
EE was removing the tail piece guard, stepped back and twisted his right ankle.
While winding belt on a pull with scoop, the scoop bucket was swing toward this employee striking his left foot.
Employee was carrying a 20# Fire Extinguisher up a flight of stairs in the shop area. The employee stated that he felt a ""pop"" in his right shoulder. The employee stated that he did not began to feel any pain in the shoulder, until the following day.
Employee was pulling a piece of handrail into the plant when his hand slipped and bumped against a piece of decking or sheeting, causing a laceration.
Employee was installing roof bolts, when the drill steel got stuck in the bolt hole in the mine roof. As the employee was reaching to pull the drill steel out of the bolt hole the drill steel fell out of the bolt hole and struck his left middle finger which was resting on the roof bolter drill pot, resulting in a fracture to his finger.
EE walked to right side above engine compartment when he slipped on the deck of the rock truck catching himself with his other foot resulting in twisting his back.
EE slipped off the roof bolter and fell to the ground, pulled muscle in lower back.
Employee reports right shoulder strain, which he believes is the result of repetitive motion of lifting/carrying during the normal course of his work. Employee has requested medical care and a claim has been accepted as an occupational illness.
Employee was exiting front-end loader using the permanent ladder stairway and handrails on the equipment when his foot slipped on the second step. Employee fell to the ground backwards, using his right arm to control the fall. Employee suffered a compound fracture of his lower right arm upon landing on the ground.
Employee stated he was Rockdusting on a conveyor belt line when he turned around to walk out of a x-cut and struck his head on a brow injuring his head/face and fracturing his top rib. He was treated at a medical center and released.
Lifting parts to clean them while working on a 988G loader, strained low back.

Employee was fueling a dozer and he states that he turned quickly to shut off the fuel when he heard a snap in his right knee and fell to the ground. He was taken to NNRH where he was diagnosed with a fracture and underwent surgery.

Employee states he was exiting the cab of a scoop when his right foot slipped in mud and reported pain in his right knee. Employee has continued to work, but with pain. Employee was taken off work on December 1 for arthoscopic evaluation and according to the operative report, a torn Chondral Flap was repaired.
Employee was installing a roof bolt. While doing so a drill steel fell out of the hole being drilled and struck him on the left middle finger.
EE hit a bump while driving haul truck and the seat bottomed out, jarring EEs back. EE was taken to the ER to be observed and taken off work for one (1) day.
Injured was taking apart belt structure and was underneath it when it came down landing on his knee.
EE was climbing a ladder to get to the top of a overcast, when the ladder began to tip over. He jumped from the ladder landing on his left ankle. He went to the doctor and x-rays revealed no fractures. EE started missing work on September 2 when he was diagnosed with ligament damage.
Serviceman fell backwards from his truck landing on his back striking his head against the ground.
Roof strap fell from the plank jack on the miner and struck EE on his right elbow.
Another contract hauler didn't have his tailgate latched when he was loaded. He, along with the injured, attempted to lift it open to allow material to fall out so that the tailgate could be latched. When the material fell out they could not hold the heavy tailgate and it slammed shut with the injured's finger in the latching mechanism.
Employee was stacking dust, picked up bag and next bag came off belt line and landed on bag that EE was picking up and shoved his right hand 2 fingers between the belt and metal plate.
#5 MINE ACCIDENT. Employee was on bulldozer and stepped down and turned too quick and felt a sharp pain in his back. EMPLOYEE DID NOT LOSE TIME UNTIL 8/18/2010 FOR TWO DAYS
EE was using a torch when an actylene hose broke behind the blow back valve causing burns to the right hand.
Was operating 345B cut excavator to pull brick rock lose and the bucket slipped off the rock jarring him back and forth causing whip lash motion causing injury to upper back area and rib cage.
Employee was cutting piece of belt with utility knife on old 2 west No.1 belt line, knife slipped and cut thigh above the right knee and lacerated leg 3 to 4 inchs long.
Working on D11 Dozer tracks. Loosening bolts with hydraulic wrench, used air wrench to remove bolts.
Using a spud bar to break up coal ring. The bar slipped and hit employee in head
EE was working on power lines. EE was doing ground work for the task and at one point felt some discomfort in his left shoulder. EE thought the pain would go away, so did not seek treatment. EE went to the clinic on 8/16, when the discomfort did not go away and was taken off work.
After starting the air compressor to use air wrench employee tossed the hose over the compressor to the other side of the truck. The employee felt a pop and pain in his right shoulder joint. After further testing a determination was made to do a rotator cuff repair operation. On November 1, 2010 surgery was performed and a tear was confirmed and repaired.
EE was removing track from the 15-W Section. The track rail was raised with a track jack and crib blocks. EE was removing a rail tie with a pry bar when the jack kicked out and the rail moved towards the EE striking the right ankle and top of the foot.
EE was being loaded and was jarred in the haul truck when some muck hit the dove tail. EE was taken off work.
Employee was struck in neck with a chip from pin as the track pin was being driven in with a hammer. Bleeding was stopped in less than 1 minute. He was transported to hospital for further exam. He was airlifted to U.K. for precautionary examination.
As the employee was attempting to hang a ventilation tube, a slicken sided piece of roof rock 7-8-inches in diameter and 4-inches thick, fell and struck the employees hard hat. The hard hat cut the employees forehead.
The employee was working in an excavator while another employee was hammering. The excavator was approximately 200 to 300 feet away from the hammer. A small piece broke away from a large rock, flew towards the excavator, hitting the employee under his left eye.
Employee operating stationary belt saw and completing a cut on a block of marble. Standing on platform at control panel, noticed movement to his left. Saw a slab tilting outward, reached to left to very edge of slab. At same time, a 4"" x 6"" x 5' timber used to hold slabs in place came loose toward employee, came in contact with tip of finger on left hand.
As EE did a belt fire boss run at 6 left drive he slipped and fell hurting his left knee.
Party was standing on the racks of a 475 Komatsu bull dozer replacing cylinder blade when he slips and fell off the dozer striking his right lower leg against the dozer requiring 11 stitches.
When getting out of a mucker, employee stepped on a rock and twisted his right ankle. X-rays at ER were negative, but he was placed on restrictions.
EE was lifting a 50lbs bag of clay when he felt a pull in his left arm and started having pain. An Orthopedic doctor diagnosed EE with left elbow strain and treated with a cortisone shot. His arm was placed in a elbow strap and he was instructed not to use it until his follow up appointment.
Employee was entering dozer when he grabbed the hand rail to pull himself onto top step when he felt his right shoulder pop.
Employee was driving through a rough spot in the road and felt pain in back. After continual treatment, employee began losing time on 09/03/2010.
While maintaining the 5 1/2 crusher employee's hand got caught between the pully and the belts. He suffered a dislocated thumb and cut finger and fracture of the finger.
EE was placing tie block under loader and smashed thumb in between block and frame of loader. Laceration to left thumb requiring surgery.
EE was backing his Liebherr Rock Truck under the Liebherr Loading Shovel and made contact with the bottom of the bucket. EE strained his neck
Employee was helping put a piece of top belt structure in. He was in the number 4 entry. As he was lifting he felt a pain in his lower back. He continued to work and due to complication he started missing work on Sept. 14, 2010.
On 8/4/2010 the employee stated that he was experiencing kidney pain. On 10/13/2010 the employee was diagnosed with a lumbrosacral sprain. The employee alleges that he injured his back while loading supplies on a roof bolter.
Injured was walking across section when a piece of rock fell striking injured in back and neck.
EE was walking past the section loading point. Just as he walked past the tailpiece he slipped on uneven bottom twisting his left knee. He was diagnosed with a torn meniscus and started missing work on 8/30/2010.
EE stated that he stepped on a pile of coal and twisted his left ankle. EE worked up until 8/16/10, notifed mine on 8/17/10 he would be off.
EE scaled a piece of rock from the top in the track entry. When he bent over to remove the rock another piece fell and struck him in the back. EE started missing work on 8/11/10. Could not contact him to find out reason for missing work. Rec. info. from Comp. on 8/20/10 that EE had been off work due to injury. After rec. this info. it is determined to be a lost time accident.
Employee removing door from bucket elevator. Attached come-along to hold door but forgot to attach the hook to the door. Took bolts off of door. Pulled door off and it fell striking employees foot.
Member was in the process of packing up material beside the section power in preparation to pull the power center back for pillaring, when a small pice of coal rib fell on the member's left leg.
Operator was dismounting and mounting a Volvo articulating truck throughout his scheduled shift. Later in the evening, EE noticed pain in his knee and notified supervisor via text message that evening and sought medical tratment the following day.
Injured was riding mantrip when it hit broken rail causing injured to hit mantrip canopy
EE was cleaning up when a piece of draw rock fell out between bolts and hit him on his left foot and fractured it.
