An unintentional fall of the mine roof was discovered at the 104 cross cut along the #3 entry of teh 7 left panel. The fall was approximately 30 feet long by 18 feet wide and 6 feet high. The fall was immediately reported to MSHA and The State by phone.

The #1 elevator was out or service for a period of time exceeding 30 minutes due to a loose connection on a resistor for the retiring cam motor. The connection was tightened and the car was tested and returned to service @ 8:35 AM. No individuals were on the elevator at the time. All agencies were notified according to law.
Mine foreman doing weekly came up on a roof fall in old sub mains at #2 Hdg. 33 to 34 blk at spad 1900. Cover is 240'. Support is 7' tension rebar. Length - 40', width - 17 1/2', height - 8 1/2'. Mined June of 2010.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to the operating vane for the leveling switches at the bottom landing being damaged. The vane was repaired, the elevator was tested and placed back into service @ 1:00 pm. No one was on the elevator at the time. All neccessary government agencies were notified according to law.
On or about 09/30/2010 an unplanned roof fall occured on the 3rd S.W. at cross cut #100 in #2 entry. The fall was approximately 18' Wide X 18' Long X 10' high. The area was bolted with 8' & 10' roof bolts. The fall restricted ventilation, and was cleaned up.
Heavy rain overcame a diversion ditch and the water ran into the mine's surface pit sump. As a precaution because of the continuing heavy rain, mine personnel were instructed to evacuate the mine at approximately 11:15 am. At approximately 01:17 pm, the heavy rain and water from the diversion ditch overcame the mine pit sump pumps and water entered the primary escapeway.
A rock fall was found at 11:10AM on the James sub maines, #7 entry, spad # 37+80. The fall is approx. 6' plus high, 90' long by 19' wide, plus in connecting crosscut four an additional 40' by 19' wide. The location is approx 3,300 feet from any working section in the return old works.
A roof fall occured in the #11 seam and is located in the 1 Main East, #4 entry, between crosscuts #6 and #7. The fall measured approximately 15'x 18'x 6'-8'. The area was supported with 30"" fully grouted roof bolts.
Intersection fell 20' x 20' x 5' high. North west mains #3 entry 3rd crosscut inby spad #37 old panel. Doesn't interfere with air or anything at mine.
ROOF FALL: A roof fall occurred in the No. 1 Entry above the 6 FT roofbolt anchorage on the No. 2 Longwall. The roof fall was approximately 15' long and 8' high and 20 feet wide.
There was an unitentional roof fall that occured in the 12I section at #2 x-cut. The fall measured approx. 12ft wide, 30ft long and 9ft high. This fall fell on top of an unattended 630 DBT scoop. ******No damage to the scoop or injuries as a result of this incident
A rockfall was found during a weekly examination at Holly Bush Submains, the fall was located at brk. 40 in the #2 entry extending thru 2 left entry. No ventilation or machinery has been affected. The area has been crib and dangered off, and additional roof support has been added to #1 entry.
A roof fall occurred in the #8 entry in return off the #1 belt. No equipment or injuries were involved in the roof fall.
Rock fall was found on the date and time stated ni item 6. The fall was at Break 10 in the neutral of #3 belt. No equipment was involved. The fall was 80' Long, 18' Wide and 3 to 7 feet thick.
A fall of the roof occurred in the South East life of Mine Mains C at crosscut 426 in the #4 entry. Ventilation, Men nor equipment were involved.
A roof fall was discovered in the 1A submain between #1 and #2 entries, xc-9 spad # 6+50. The fall was length-20ft,width-19ft and height 6ft plus.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to a bad power resistor for the retiring cam. The resistor was replaced and the car placed back into service @ 7:15 PM. No individuals were on the elevator at the time. All escapeways were re-routed and the necessary agaencies were notified according to law.
Roof Fall in Northwest Mains #2 Entry (2 breaks inby spad #130) measuring 5' thick x 100' long x 16' wide.
A fall of roof was discovered in the 1st South East Mains in # 4 cross -cut in between #2 and #3 Entry. The fall was approximately 18ft long x 18ft wide x 8ft high.
A roof fall was found in the # 4 entry 1 crosscut inby survey station #446. The fall measured approximately 75' by 20' by 4' and does not effect the mines ventilation.
AT 7:00 PM 09/27/10 A ROOF FALL WAS FOUND IN THE MAIN WEST INTAKE ENTRY # 8 1 X-CUT OUTBY ZERO X-CUT. ALL ENTRIES TO THE FALL WERE PROPPED AND CRIBBED.
The continuous miner was cutting in the #8 heading of the 7A Northeast Mains when it breached old works in the right hand corner of the face. The old works had been mined in 2004. Water entered the active section through the hole. The power was disconnected and the men immediately evacuated the section to the surface. MSHA was notified at 4:07 pm.
During a fireboss run a roof fall was identified in the #8 neutral entry inby spad #6997. The fall was approx. 50' L X 20' W X 4'-5' H. The area was supported by 4' fully grouted resin bolts. The area was mined approx 5/06. Cribs have been set and the area dangered off.
A roof fall was discovered at 6:05a on the B-2 Panel (#3 Heading in #5 Crosscut, 600' outby the loading point). The area was bolted with 6' SuperTwist bolts and 12' Point Tension Cable Bolts. No ventilation or equipment was disturbed. No one was injured as a result of this fall.
An unplanned ignition occurred on 9/26 at approx. 11:45a.m. on the 8th west headgate working section, entry #3, MMU 009. MSHA was contacted via One-Call-Does-It-All.
The man hoist stopped functioning at 11:00 p.m. due to a brake motor malfunction and was back in operation at 5:42 a.m. on 9/27/10 when the test run requirement was completed. The man hoist malfunction had no effect on the primary or secondary escapeways.
A fall of roof was discovered in the 3K Panel, in the #5 cross cut, in the #5 Entry. The fall was approxiamtely 75ft long x 20ft wide x 6ft - 8ft high.
On the 2nd right main, 2600 ft underground, a roof fall occurred in #1 and #2 Entry, blocking roadway. No injury to anyone.
Weathering and scaling of the pillars made area on top of pillars to small to handle roof load. An area of about 9 rooms fell out that was approx. 6' thick.
Lightning struck wooden power pole on mine property. Mine inactive at the time of incident. Mine supervisors present on property found the pole burning and notified MSHA. Not able to extinguish fire within the required time limit. Water truck available but power company not able to deenergize power lines for over one hour due to location of the property.
An unintentional roof fall occurred at portal 6 seam west main entry #1, intersection at 1790 west. The fall was approx. 40' L X 18' W and approx. 12' thick. The fall did not affect passage of men. The fall will not be cleaned up. Fall #27P
The lilly controller on the 7 Shaft hoist malfunctioned. The contacts were repaired and there have been no further problems.
A rock burst happened during the afternoon shift blast. it was discovered by the graveyard shift boss and reported to MSHA. The area is question was about 8 feet long by 4 feet wide. It knocked out one 4 foot bolt but did not go beyond the 8 foot anchorage zone.
The area was a wet laminated top and was in an intersection approx. 50' L. X 30' W. and 3-5' thick. It fell in the entry between #3 and #4 belts one break outby spad #1715. This fall does not block any travel or escape ways. We will probably clean it up or go over the top of it, it does not effect ventilation.
A mining supervisor noticed a fire in the D 1 area behind the pump barn where oil and gas workers had been mowing pipeline right of way. The fire was put out within 20 minutes of being seen. No Miners were working in the area. This did not affect the Oak Hill mining operation. Estimated 5000 sq ft of underbrush was affected.
A roof fall was discovered in the Main West #1 Main, return air course, in # 3 entry, at crosscut # 45. The roof fall measures approximately 20' long, 19' wide, and 8' thick.
An unintentional roof fall was discovered at 4:22 on the Southeast panel at break 16 at spad #0629. The fall measured approximately 4' high X 16' wide X approximately 40' long.
Miners were working on face ventilation in 4 North when methane began to spike. The power was shut off to the section and MSHA called.It was later found that the return overcast for the section had been damaged from overflowing water from a pump station losing power to the water pump.
An unplanned roof fall occurred approximately 5000' from the active face at survey spad #60381 in the return entry. The approximate size of the fall was 250'x18'x9'. No injuries or equipment was involved in the fall.
An unintentional roof fall has occurred at the 18xc of the Main East in the #10 entry. The fall was approximately 20 feet long and 17.5 feet wide and 9.5 feet high.
Roof fall occurred on 2 south mains 2nd left break 27 in the belt entry. Fall was 20' x 20' x 8' thick. Roof control was 6' torque tension bolts.
A roof fall was discovered in the Main West #2 Mains, return air course, at 97 crosscut, between # 2 and # 3 entries. The roof fall measures approximately 30' long, 18' wide, and 6' thick.
A ROOF FALL WAS FOUND IN THE BLEEDERS OFF SMOOT CREEK SUBMAINS THAT MEASURED APPROXIMATELY 55' LONG, 20' WIDE AND 6' TO 8' THICK.
Roof fall discovered in entry adjacent to primary escapeway.
At 2:00 a.m. a roof fall was discovered by an examiner in the old works of old #4 unit in the # 9 entry of #6 panel near the last cut thru from the 5th to 6th panel. There were no injuries or witnesses. Also no disruptions to ventilation or passage of personel.
Fall was found at 1 west mains, 25 break, spad # 382. Fall was 80' long x 19' wide x 7' high. roof support was 5' and 6' torque tension bolts.
An unplanned roof fall occurred in the #2 entry of the old Headgate #2 section, this fall occurred at spad #4451, in the #2 Entry. Ventilation and passage of persons was not affected. Fall measures 80'L X 20'W X 6-8'H. Area was supported by 48"" resin Bolts. All entries have be breakered off. State/Fed agencies notified.
At 1:00 a.m. a roof fall was discovered by an examiner in the Main West Intake at xc 160 between #7 and #8 entries. There were no injuries or witnesses. Also no disruptions to ventilation or passage of personnel.
The liner on the skip door had come loose fouling the door. The men were evacuated from the mine and repairs were made, men were allowed to reenter the mine after all working places were examined and cleared. M.S.H.A. was notified on the hot line at approximetly 7:00 a.m. to notify them of the problem.
After finishing the final cut in the #6 entry and cleaning up, an unplanned break through of water came into the section from the adjacent panel and ran into the last open crosscut and remaining 3 entries. All eq. was pulled back and power to each was knocked.
An unplanned roof fall occurred at #8 bck on the #4 belt and #7 entry that measured approx. 50' L X 18' W X 7' H. No injuries were reported. The area was developed on 2/27/09.
An unplanned roof fall occurred in the #3 Entry at spad #4408. This fall is 6 Breaks inby #9 Belt Head. Area was developed 3/2010 and was supported with 4 foot resin bolts. Fall measures 25' long x 20' wide x 6' high. At this time mine is in a non-producing status. Federal and State Agencies were notified immediately.
During the preshift the foreman found a roof fall in the # 9 heading from 2+00 to 4+00. The fall measured 200' X 40' X 8'. There were 6' resin bolts used and the fall occurred at 8'.
Unintentional roof fall in No. 4 entry on No. 2 belt (secondary escape way) at survey station No. 463. Sides of fall were slick sided and appeared to be horseback in roof. The mine was in a non-producing status and under going a re-opening inspection at time of fall.
A rock fall occurred above anchorage at crosscut 14 between #2 and #3 return entries along the #1 belt at spad #176 The fall knocked out a return stopping. No one was injured or trapped, no damage to equipment. The return air course was rerouted.
AT 1:10 AM A ROOF FALL WAS FOUND IN THE MAIN WEST INTAKE ENTRY # 8 X-CUT 118.
A suspected methane ignition occurred in the #6 entry face on the 2 left off North Main section. The miner operator described seeing a orange flash as he was attempting to cut cap coal from the roof.
A coal seam fire started due to blasting in the area of job #4 K cut reclaim. The fire is not close to anything that can be damaged and poses no threat to miners.
At 7:22 a.m. a roof fall was discovered by an examiner in the Main East at xc 83 at old 4A header between #5 and #6 entries. The fall measured 18' W X 30' L X 8' H. There were no injuries or witnesses. Also no disruptions to ventilation or passage of personel. The Main East Belt Line was affected.
A roof fall was discovered in the Main West 1, at 60 crosscut, between # 5 and # 6 entries. The fall measures approximately 40' long, 18' wide, and 6' thick.
AT APPROX. 5:00PM 09/20/10, A ROOF FALL WAS FOUND IN THE NORTH SUB-MAIN INTAKE ENTRY #7 X-CUT #21.
A rock fall was found at the time and date stated above 13 Breaks inby the Portal between the #2 and 3 Entry (crosscut) during the weekly examination of this area. The size of the fall was 20' Long, 18' Wide, 2-3' Thick.
An unplanned roof fall was encountered outby the dumping point of the number 3 section out by survey station number 4317 in the intake split of air used to ventilate the setion. MSHA was notified at 08:20 am on 09/20/2010. An AR of MSHA came to investigate on 09/23/2010.
A roof fall was discovered extending from the inby corner of 10-58 xc. outby to the inby corner of 10-57 xc. Fall is approx. 15.5' high X 15.5' wide X approx. 50' to 60' long.
The feeder breakers tripped out at the surface TIR substation which feeds the mine's dewatering system and # 1 secondary hoist. The breakers were reset and held.
Fall was found on 2 south mains #8 entry inby spad # .777 from 35 break to 36 break. Roof control was 6' torque tension bolts. Fall was 120' long x 20' wide x 8' high.
The # 1 secondary hoist electrical feeder cable was found to have a defective splice. This splice was what caused the breakers to trip 09/18/10 at 9:00 pm. #1 secondary hoist was down during repairs of the feeder cable and installation of new pole line fuses.
A fall of roof was discovered in the 1st South East Mains in the #7 Entry in the # 12 cross - cut. The fall was approx 20ft long x 20ft wide x 9ft high.
At approximately 8:30AM the #2 coal mill system lost coal feed and the coal mill explosion vents were ruptured.
A ROCK FALL OCCURED ON THE #10 BELT APPROXIMATELY 18' WIDE 65' FEET LONG AND 6' HIGH. THIS FALL WILL BE CLEANED UP AND SUPPORTED.
During the weekly examination, a reporatable roof fall was found in E10 Headgate, entry 2, crosscut 2. The fall was approximately 20 feet wide, 20 feet long and 12 feet high.
While examining an area on Old Zippo Mains where a preposed bleeder was to punch through a roof fall was identified. The fall was approx. 65' L X 20' W X 4'-5' H. The area was mined approx. 1/11/07 and was supported with 4' fully grouted resin bolts. The area has been dangered off and all approaches have been timbered.
An unintentional roof fall was discovered at the 105 break in the #4 entry south mains, fall was estimated to be 30'Lx3'-4'Hx18'-19'W. The area had been supported with 4' fully grouted resin bolts, no injuries, no equipment caught, ventilation was not interrupted. Fall will be cleaned up this mine is in a non-producing status.
IGNITION: The ignition was in the face of #1 Longwall at #122 Shield. The fire was in the top on the Headgate drum. The Shearer was going to the Headgate and had stopped and was going to go back to the Tailgate when the ignition occurred. The flame stayed on the top corner where the roof meets the fact. It was put out with water and 1 fire extinguisher.
At 12:45am the hoist was not working properly. At 12:47am EE examined the hoist and changed a battery. At 12:48am EE called MSHA with the information. At 1:22am the hoist was back in operation.
The #8 elevator was out of service for a period of time exceeding 30 min due to the governor wire rope used to engage the safety catches had a diameter reduction exceeding 6% for approx 28"" in length. No individuals were on the elevator at the time. All affected escapeways were re-routed. The cable was replaced & the elevator was back in service @ 6:58 PM.
A roof fall was discovered in No. 2 entry of the left return aircourse near spad 626. The fall measured approximately 20'wide by 80'long x 7' thick. There were no injuries and no damage to equipment.

A MSHA inspector was with me upon discovery of the fall and participated in the investigation. The roof and initial area ground the fall appeared to be in fair condition. An EE was grading the haul road and was approaching the pit where our 345B trac hoe was working, he noticed that fire was coming out of the engine compartment. He informed the operator of the hoe and the hoe man stopped the engine and dismounted. There was no injuries.
The above mine has experienced a non-injury un-intentional roof fall outby the working sections in #7 entry 1 x-cut inby 003 sections return overcast. Laminated shale deteriorated and gave way. Fall demensions; 20' wide, 30' long and app. 8' to 10' thick.
A roof fall was discovered at approximately 10:55 AM on our #2 Belt. It measured approximately 6'H x 10'W x 30'L. All agencies have been notified.
An unintentional roof fall was found 4,300 from the 003 section in the #6 entry intersection on the southwest mains. The fall was cribbed off. the size of the fall is 40' long x 20' wide x 6' to 8' thick. It is located at or near spad # 9634
A fall occurred at 13-M #1 entry at D5622, ventilation to the #1 seal is obstructed as is the walk way. Fall will be re-supported by post 1 cribs to re-establish the walkway and vent to the seal. No persons affected by fall.
A Fall of roof was discovered in the 3K Panel at cross cut #32 in the # 8 Entry. The fall was approximately 20ft long x 28ft wide x 6 1/2 - 8ft high.
An unplanned roof fall occurred in the Coalburg mine at the #6 brk on #16 belt in the intake that measured approximately 30' long x 20' wide x 4' high. No injuries were reported. The area was developed on 6/17/08.
A ROOF FALL WAS FOUND AT X-CUT 99 OF THE MAIN WEST TRAVELWAY. ALL ENTRIES TO THE FALL WERE PROPPED AND CRIBBED. CLEAN UP HAS BEGAN PER OUR APPROVED CLEAN UP PLAN.

A MSHA inspector was with me upon discovery of the fall and participated in the investigation. The roof and initial area around the fall appeared to be in fair condition. An elevator was stalled in the Step 3 Concentrator for approximately 90 minutes.
Roof fall occurred at 30 crosscut #2 to #3 entry of the South Mains. The fall is approx. 30 ft long by 20 ft wide and 13 feet high from mine floor. The fall did not impede travel, did not impede ventilation and did fall above the anchorage point. The area was roofbolted with 8 ft. resin assisted, metal straps, & screen.
Coal was smoldering and was spotted at approximately 12:35 AM, steps were taken to extinguish the smoldering coal.
A Cat 16H motor grader was blading the West Waste Dump area as a loaded Cat 789C haul truck approached to dump. Just prior to stopping forward motion, the haul truck contacted the motor graders front left wheel.
An unplanned roof fall occurred at spad #20079 off the #1 section. The fall was about 80 feet long, 20 feet wide, and 10 feet high. The area was bolted with 5 foot torque tension bolts. The fall will be timbered and dangered off and will not be cleaned up.
A roof fall occurred in the #3 heading of No. 4 Northeast of the MMU 002-0. The fall was in the last open cross cut at survey station No 3302 and measured approximately 20' wide by 25' long by 6' plus high. There were no injuries.
An Unintentional roof fall occured in the 15H longwall section. The fall occured in the intake intry between 35 and 36 x-cuts. the fall measured approx. 65 ft long, 9 Ft High and 16 ft wide. *** No injuries or equipment damage as a result of this incident

A roof fall occurred in the 2nd West Submain in the #3 entry between spad number 27+70 and spad number 28+40 measuring approximately 34 ft. long x 19 ft. wide x 9 ft. high. The area was supported with 4 ft. glue bolts, 8 ft cable bolts and square cribs.
A roof fall occurred one break inby survey station 25895 in the #6 entry of the 6A Northeast Mains. The fall was approximately 60' long x 20' wide x 8' thick. The fall is approximately 1600' from the working section.
An unintentional roof fall was found in the #3 entry netural of the 2nd south west mains, 4000' from the 003 working section. Fall was breakered off with cribs. Size of fall, 20'X30'WX6-8' thick. Spad #9710.

A mechanical problem with the SC4 conveyor head pulley resulted in the conveyor belt catching fire. A plant supervisor shut down the conveyor and local Fire Department was called to extinguish the burning conveyor belt. A limited amout of damage (36 feet of conveyor belt) and no injuries as a result of this incident.
During the examination on 9/10/10 at apporx 5:00am, a roof fall was discovered by EE and another EE, WVOMSH&T. The fall is located in the crosscut between 3 and 4 entries, near spad 679. The approximate dimensions are 20 feet wide X 25 feet long X 4 feet thick. The fall has been breakered off.
Roof fallat 32 & 45 B entry, west B return.Size of fall was 8' High X 25' Long X 20' Wide. Ventilation was not affected.
An unintentional roof fall occurred 8,000' from 001 main 20'X60'X8"" high (cribbed off all sides) found on exam. Bolted with 6' fully grouted tension rebar. 1st n.w. mains spad #93 to #94 in between 1 and 2 entry.
A roof fall above anchorage occurred in the No.6 entry adjacent to the belt entry. Fall measured approx 7' X 18' X 20'. 3600 foot outby Jenny branch section.
A fall of the roof occurred in the South East Mains in the # 4 entry at crosscut 181. No equipment, ventilation nor personnel were effected by this fall. This fall is more than 8 miles away from the closest unit.
Rock fall in 1-left panel off of 1-east sub main in #5 entry at spad No. 7287. Rock fall fell in x-cut went inby for 30' outby for 35', to the left for 50' and to the right for 30'. Fall liiks like it might be 8' to 10' high. Rock burped real small pieces. No vent interupted in this area. We don't intend to clean this rock fall up.
We experienced a rock burst at blasting time of the day shift. It was discovered by the night shift and reported to MSHA via the 1-800 Number. The affected area of the rock burst measured 6 feet high by 18 feet long by 4 feet wide. The area was secured until the inspectors arrival on 9/11/2010.
Shaft work was in progress, hoist drive kicked out due to GFI. Was reset and kicked out again. Shaft workers stuck in shaft for over 1 hour while drive was troubleshot and enough operational capacity given to bring the workers to surface. Later it was found that damp weather conditions had resulted in condensation of moisture in and around the hoist motor ventilation system.
A roof fall was discovered measuring approximately 30'L x 20'W x 6'T in the #3 entry at 99 break on 14 headgate section.
An unplanned roof fall occurred at spad 248. Fall measured 100' Length X 20' Wide x 5' High. No Injuries were reported.

Examiner discovered a roof fall approximately 18'W x 60'L x 10'H in the 8 Haulage bypass at the #14 crosscut between #4 and #5 entries.
An unplanned roof fall occurred in the #2 gas mine in the #4 entry 30' inby spad #1290 that measured 20' long X 19' wide X 14' High. No injuries were reported. The area was developed on 1/31/08.
A roof fall was discovered measuring approximately 100'L x 20'W x 6'T, in the #1 entry from #1 to #2 break at the cut thru EP.
An area near a previous roof fall had been cribbed the rock shifted causing the cribbing to fall. No damage was done and travel and ventilation was not affected.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to a problem with the retiring cam cable. The cable was changed and the car placed back into service at 7:30 a.m. No individuals were on the elevator at the time.
Fall was found on 2nd left off 2 south mains at 24break. Roof fall was 6' High x 40' Long x 18'Wide.
The man/material hoist became inoperable due to a malfunction with the counter weight. A guide had worn out on the counter weight and the counterweight came out of the rails that contain it in place. The guide was replaced and the hoist returned to operation at 9:20 AM.
Roof Fall inby spad #2727 (60'L x 20'W x 4'H). The section was moved back 5 crosscuts.

a roof fall occurred above the anchorage,outby the No 2 section, in the No 6 entry just inby survey spad 3223, just outby survey spad 3215. the fall measured 20 ft wide and 60 ft long and 5 to 6 ft high. There was no ventilation interruption nor did this fall affect the passage of men. The area will be cribbed and dangered off.

An unplanned roof fall has occurred at or near spads 4307 & 4442. These are worked out ares of the mine and the falls were found during weekley examinations of airways. No injuries reported. Area developed in 2003.
A bearing on the 1West belt tail roller overheated resulting in a fire.
#1 salt hoist pinion brake failing to release properly causing the hoist to 'kick out'. The hydraulic pump was changed and the SOBO valves cleaned out.
An unplanned roof fall occurred on the 011-0 MMU on section #2 in the #2 entry about 480 feet from the face. The fall was about 640 ft long, 18 to 20 ft wide, and 10 to 12 ft high. The area was timbered and dangered off. This area was bolted with 5 and 6 ft torque tension bolts. The section moved to new set-up.
A roof fall occurred on the mainline track at survey station 32+00. The fall is approx. 25 ft. long by 18 ft. wide and 10 ft. high.
Employee in the DII Mining Area noticed smoke in the DI Mining reclaim area. The supervisor investigated the area and noticed a fire burning in the tree farm near 782 highway. The water truck and local fire department extinguished the fire in one hour. CAUSE - Wind re-ignited embers from the 8/25/10 grass fire in the same area.
The #1 elevator was out of service for a period of time exceeding 30 min due to the bottom doors being left open. No individuals were on the elevator at the time. The doors were closed and the elevator placed back into service at approx 6PM. The report of the outrage to the federal agency was in progress as the elevator was placed back into service.
The #8 elevator was out of service for a period of time exceeding 15 min, with 1 individual on the elevator at the time, due to a bad eurotherm drive. A plan was put into place to bring the individual to the surface @2:44pm. The eurotherm drive was replaced the elevator was inspected and placed back into service @ 4:39PM on 9/5/10. All agencies were notified.
At approximately 2:00 pm smoke was noticed coming from the north end of the Q area. The onshift supervisor investigated and called the local fire department, MSHA was notified around 2:20 pm. A call later from the onshift supervisor verified that it was not on mine property and had been caused by a tree falling over striking a power line causing a grass fire.
Roof fall occurred on the 6 North belt at 23 block intersection. Ventilation was not disrupted, walkway was obstructed. The fall area was dangered off on all approaches. This area will be cleaned and supported with bolts and arch supports. No person affected by the fall.
A D9 Dozer caught on fire for undetermined reasons on the surface area of the Surface Facilities. No one was working on this shift at the time of the fire. The fire was discovered by a dispatch person that works at the underground mine location.
A fall of roof was discovered in the 1st North West Mains at x-cut #39 in the #1 entry extending over to #2 Entry. The fall was approximately 75ft long x 18ft wide x 6 1/2ft high.
A unplanned roof fall was dicovered at 10:10 a.m. on 9-3-10 by the Mine Foreman in the # 5 zone return at # 11 blk in the 2-1 crosscut. There were no injuries and the fall did not block an escapeway or a regular travelway. All approaches to the fall have been dangered off and timbered and additional timbers have been set around the fall area.
An unintentional roof fall occurred at the Galatia Mine new future portal 6 seam west mains entry #2 1880W. the fall was approximately 46 feet long, approximately 18 feet wide and approximately 10-12 feet thick. The fall did not affect passage of men. The fall will be cleaned up. Fall #270
The #9 LHD caught fire while tramming to the 23H North face. The fire started in the engine compartment. The cause of the fire has not been determined. The ee stated that he turned off the LHD's engine & activated the machine's fire suppression. It did not extinguish the fire. The fire was too intense to fight with hand held fire extinguishers. The LHD was destroyed.
West motor blower experienced an electrical short in a junction box.
A rock fall was discovered in the #6 entry between #28 and #29 crosscut one break inby spad #501. Size of fall was estimated to be 30""L x 18'W x 9'-10'H. The area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will not be cleaned up.

Mine examiners discovered a small fire at crosscut 7 on North Main 3 belt which was extinguished with fire extinguishers and fire hose.

While inspecting the coal storage pile, a small flame was observed along the outside bank of the pile. No injuries occurred, No flames observed while uncovering this area. This has been reported to the field office.

Truck haulage accident involving two 100 ton rear dump trucks. It appears the victim did not observe the stopped truck and struck it in the rear.
In the process of reclaiming high voltage cable,the high voltage cable was being pulled with a cloth rope and shive wheel. The injured employee put his hand on the rope while it was being pulled and his right little finger was pulled into the shive wheel. This resulted in his right little finger being cut off at the last joint.
Employee was replacing shield on a cutter head when he dropped the shield & injured his left hand.
Employee was attempting to hook up a Highwall miner dolly to a truck. The tongue broke loose, fell to the hitch, causing his right hand to be pinched between the tongue and hitch of the truck. He was admitted to the hospital and on 9/23/2010, surgery was required to amputate his right index finger.
Employee was trying to hold curtain out of the way so he could see to scoop area and caught his fingers between the canopy and a damaged roof bolt, injurying his ring finger and amputating his little finger.
Employee was bolting in the 3R crosscut on NO. 7 unit. MMU 011-0. After installing two bolts he was struck by a piece of roof rock fracturing his back.
Working on the drive of a screw conveyor. One employee was jogging, when another grabbed the belt and tried to manually jog. His thumb was caught by the belt and his right thumb was amputated.
Worker was trimming slate when he moved right hand and part of right hand caught under the blade, causing the right ring finger and small finger to be cut through. Gloves were worn.
Employee was installing roof bolts and as he was reaching to remove the drill steel, a rock (approximately 18"" x 14"" x 4"") fell from the mine roof and mashed his right index finger against the roof bolter drill pot, resulting in a laceration and crushing injury. His right index finger was amputated at the middle phalange at the hospital as a result of his injury.
EE was lifting a coupling on a rock dust push out car to hook it up to a Getman Tractor. His right ring finger got mashed between the two couplers, mashing the end of his finger off.
He was a passenger in a battery jeep along the haulage. A collision with another jeep occurred. During the collision he grasped the back of the seat; the battery slid and caught his right ring finger between the battery and the frame of the jeep. This resulted in a several laceration to his right ring finger.
Operator had left hand on canopy jack while lowering canopy. His left pinky got smashed between cylinders.
Employee was helping with maint. in the Prep. Plant and had done several little things. He stated he had unhooked a small set of come-alongs and removed them. He felt some pain in his stomach. He went for medical treatment and was told he has a Umbilical Hernia. These are there from birth and can come out anytime. This must be a illness.
Slid rod off of rod rack and smashed finger between the rod and the rod chucked on the drill.
EE was helping drill operator ee in changing bit and driver on Beetle Drill. The pipe wrench was to hold the steel to break the hammer loose to change the drill bit and driver. In doing so the operator moved the control lever to hold the pipe wrench in the opposite direction and the wrench hit ee on left leg above the knee.
Employee was helping splice belt and while attempting to square the edge of the belt, knife slipped and cut right leg above knee (thigh).

Employee was lifting miner cable to place in cable hanger when he felt a pain in the left side of his groin. EE stayed on the job and came to work the next 6 scheduled shifts. Went to doctor on 10/7/10, diagnosed with hernia and began to miss work.
Employee was lifting a ladder as he turned with the ladder that he was carrying he felt a pop in his back. Received notification on 10/4 that he had attended a clinic appointment and had been authorized off work by doctor.
Miner tripped on his own shoe string and fell. Dislocated right thumb. Took him to the Clifton Medical Clinic and they relocated thumb and put in stitches.
While loading pin supplies in a scoop bucket, a rib rolled off onto his right foot/ankle.

The company sent the employee to an emergency room and the Doc. told him it was likely a rotator cuff tear. The employee took the next shift off to see a specialist. The employee stated that this was an aggravation of a pre-existing condition.
Employee stated that while traveling up the ramp with a load of rock, the truck leaned back and he injured his shoulder.
EE was preforming inventory at our warehouse and was walking across a deck plate that is used to unload semi trucks when she stumbled and fell.
Employee was hanging a reflector to mark an unbolted intersection. He was standing between the third and fourth row of bolts back from the intersection and a rock fell from between the bolts striking him in the head and back. The rock measured 35 inches wide by 80 inches long and was approximately 2 inches thick. He was diagnosed with two compression fractures in the lower back.
Employee was driving a golf cart from the working face of the mine to the lunch room when making a turn the cart rolled and employee fell out and impacted shoulder to the rough salt floor. Employee suffered a left shoulder contusion.

Injured employee was ascending the steps from the basement of the plant when he tripped and fell injuring his left hand. X-rays revealed a fractured bone behind his left little finger. Employee was excused from work by treating physician until further notice.
Height of fall and rainy weather conditions.
While reaching for a split bolt to load onto bolter carosel, employee felt a pop sensation in left knee area. The knee continued to pop throughout the day. Over night the knee became swollen and very sore.
The employee was operating a loader moving chunks for the chunk breaker operator. As the employee was backing up, debris flew through the open window of the loader striking the employee in the left eye. He was evaluated, surgery was performed and taken off of work on 9-29-10.
EE was dis-connecting the motor leads from a set of resistors inside a belt drive starting box (a tight work area)in doing so he strained the ligaments in his right hand.
He bent over to pick up a one-hole-board when he felt a pain in his lower back. He was diagnosed with acute lumbar strain.
Employee was using woma gun on the feed shelf to clear buildup in the tower. Coating (buildup) fell, hitting the gun barrel lifting the employee off of his feet causing him to fall into the scaffold when landing. He noticed a popping feeling in his right hip making it difficult to walk.
Grinding a piece of metal 3/8"" diameter off return roller guard. Metal shavings got in his left eye. He did have on safety glasses at the time. Metal shavings probably got in eyebrow and when removing safety glasses, got in the eye.
Employee was bagging lime and he tapped the bag and the bag ripped and lime blew in his eye, causing lime burns.
Employee was starting to drill with steel in the mine roof when an undetectable piece of draw rock fell measuring 12X8 thick fell vertically with the end striking his right foot behind steel toe. The area employee was bolting in was 6 high. Injury resulted in fracture to the 4th metatarsal on his right foot.
The employee was putting glue in his glue box opposite the operator. When the operator started to tram the bolter forward, he caught the man's foot on the oposite side between the tires of the bolter.
EE was building a stopping between #5 and #7 entry. A block was stuck in the mud and when he tried to pull it out he felt pain in his back. He was transported to ARH in Beckley. Tests were negative. He returned to work until 10/4/2010 and went to a different doctor who has him off work.
Employee was walking from outby on No. B Belt back to No. 1 LW. A rib rolled in front of her and she stepped on part of it causing her knee to twist.

Injured employee was walking the roadway on #1 section when he stepped in a rut and twisted his lower back. He saw his primary care physician 9/28/10 and was referred for an MRI on 10/18/10. On 10/21/10 MRI revealed bugling disc in his lumbar region of his lower back. He has been excused from work until further notice.
Employee stated that while shaking out dust from a rag after wiping down truck dashboard, the tip of rag hit him in his right eye causing abrasion.

On 10/6/10 EE notified sup and sr. sup during a meeting that he had an open Workers' Comp claim for his back. EE alleges that he injured his back on, but is unsure as to the specific date, but says that it occured on one of his dayshifts Sept 22-24, 2010. EE was also not sure as to what event caused his back pain .... This is under investigation ...
EE stated he was working on a shield, he slipped and fell landing on his right arm and ribs.
Employee was pulling on an air hose and slipped in the middle of the haul road and landed on his left hip causing a contusion. On 9-27-2010 saw Doctor and was referred for an MRI. MRI was given on 10-20-2010 and results were obtained on 10-27-2010 and were negative. This is now a lost time reportable injury.
Carrying two 5-gal. buckets of caustic to recharge tanks, while walking up stairs turned & felt knee pop. After review of medical documentation on 11/15/10 it was determined that this was a work related injury.
EE was loading pizza pans on the roof bolter, felt pain in groin area on Saturday, went to Doctor on Monday and returned to work on Tuesday.

On September 24 2010 a serious accident occurred at 10:15 a.m. at my mine when employee lost control of a front end loader, causing it to rollover. I am not aware of how it happened because I was not there at the time because I was working at another mine. He was injured severely and was hospitalized. The loader rolled over and came to a rest on its top.
While straightening out roof bolt in hole,he fell catching himself on boom, hurting shoulder. **started missing work on 10-19-2010***
Member bent over to remove cap coal that had fallen onto his neck, while an approaching shuttle car was turning toward right handed entry, the shuttle cars rearend left side bumped the member.
EE was climbing up the steps of a 777 Cat Truck (approx. 6 ft) when he fell backwards landing mostly on his left foot. He stated when he hit the ground it bent his leg backwards causing a fracture to his left leg.
EE stated that he used the heel of his left foot to push a crib out of the way using a backward motion. The EE felt pain in his left leg.
The injured employee was operating a haul truck in the pit. As he descended towards the bottom of the ramp, he lost control of the truck and traveled off the roadway striking his forehead against the windshield.
EE was working in a panel on #304 Jeep in the shop. He put his hand on the seat to stand up and the seat turned over pinching his right ring finger causing a laceration and fracture.
Employee had moved roof bolter in 3 left to let the continuous miner back up. When a piece of draw rock, paper thin to 2"" thick by 2' wide and 3' long fell and hit him on his back causing abrasions and contusions and 1 cracked vertebra.
Employee was stepping up onto a continuous miner, when he lost his balance and fell backwards, landing on a section foremans walking stick. Diagnosed with a fractured coccyx (tailbone) and right axilla abrasions.
EE was welding floor, a worker welding out of buggy was pulling up a 9"" grinder by the cord when it seperated from the extension cord falling and striking EE in the shoulder blade.
EE was rock dusting #7 beltline. EE was moving the hose under the belt to cover the offside. EE backed up and the hose became twisted and hit the fresh waterline. Dust hit his face and got into his eyes. He said he was wearing glasses. He came out and drove home before anyone could investigate. EE said his corneas were scratched. Doctor reports have not been rec. to confirm.
EE was moving charger with fork lift when a fork fell off. He said he tried to replace the fork and dropped it on his foot. He continued to work until 11/8/10 when doctor excused him from work until he gets an MRI. The forks were checked by electrician and he found the forks locked in place and they could not fall off. New forks had been recently installed.
He was getting into a jeep, he put his right leg into the jeep and when he stepped up he twisted his left knee. He continued to work at his regular duties until 11/9/10 when he was removed from work for surgery to repair a torn meniscus of the left knee.
EE SAID HE SLIPPED OFF THE LOADER WHILE FUELING HE MISJUDGED HIS FOOTING ,FELL HITTING HIS HEAD ON THE MACHINE

When reaching down to pickup lumber to load on cage, miner was bitten by a rattlesnake that was hidden under the shaft collar boards.
While moving the left roof bolter, employee was crawling and hurt his left knee.
Employee was loading up some 2x4's & when one of them struck his knee causing pain. He said it felt like his knee locked up. Then when he was done loading, he stepped down & his left knee gave out.He went to medic & reported it. But the pain has been on & off since then. So he went to doctor, and that resulted into surgery, which took place on 12/9/2010
Employee was building a brattice and fell over a block. EMPLOYEE DID NOT START LOSING TIME UNTIL 9/30/2010 DUE TO INCREASED PAIN AND SURGERY.
As he was roof bolting, injured was holding a bolt board straight while pushing a bolt up and caught his left index and middle fingers between the board and the roof amputating the tips of both.
EE was making his weekly examination along the 3rd North panel and found a 6x6 timber in the roadway. While moving the timber out of the roadway, he felt a sharp pain in his lower back.
EE was tramming scoop through water hole, timber came in deck hitting left leg. This report is being generated under protest to terminate citation #9006080.
Operator had a virus with nausea & diarrhea. The reason this report is late we were trying to find out from the Dr. if ee was off work because of the bump on his head or the virus, we could not get an answer.
While the employee was climbing the ladder on his haul truck, the employee said that his hand slipped and he started falling, landed briefly on his feet, and then fell to the ground on his back, rolling over onto his left shoulder.
Employee was washing his loader off at the end of the shift and slip and fell on the ground, falling on his right arm and fracturing his elbow.

Employee sought medical attention on 9/23/2010 for some respiratory distress he was experiencing. He had mucked in a recently blasted stope for a period of time the day before with a strong odor of ammonia. Employee was taken off work for one shift and returned full duty on 9/27/2010.
Employee states he was calibrating a bag packer weight scale with a 50# weight; the weight fell off the spout hitting his knee. The 7000-1 is being filed now because Safety Dept. became aware on 10/26 that the lost time was related to work.
EE was loading rock dust into a wet duster, when a section of the coal rib fell off from near the roof striking the EE in the head, neck and shoulders. The EE complained about pain in his neck, back and shoulders.

Employee was going to fix a leaking transporter line. He was waiting for the line to depressurize when the line came apart blowing wollastonite all over him and into both eyes.
EE was trying to straighten a 6 foot torque tension bolt to push up into the hole, when he felt a something pull in his lower back.
Employee was jacking a chain hoist when the chain broke causing him to jerk and strain his right shoulder. (Employee did not lose any work days until he was taken off work on 11/29/2010 by Dr.)
Employee was attempting to remove a double tire off ram car with a 3/4 ratchet and cheater bar. The ratchet slipped off and struck his left leg/knee. Employee started missing 01-25-11.
Employee was helping move the miner from the #5 right face to the #8 heading. Once the miner reached the face, the employee discovered that he needed more cable slack and was sheaving it around the corner using a roller hanger. The hanger came loose and the slack struck the employee throwing him to the ground. He was diagnosed with a mild concusion.
The ee was carrying a blower unit for the auxiliary fan toward the working face the mining crew was preparing to pull miner cable. A water hose connection caught one of the cable hangers causing the hanger to pull loose. The miner cable fell and struck the employees hard hat. The MRI & CT scans were negative, ee was kept overnight for observation & released.
EE was ascending 4 step fixed ladder to machine platform of DM45E Drill - in reaching at an approx 45 degree angle for right handhold, EE felt pain in right shoulder
Employee was moving Lotrac and ran over 4"" water pipe and the pipe swung around and struck the employee on the right ankle.
EE was lifting a rock that was hung between tailpiece and feeder and felt pain in the groin area. His first day missing work was 9/28/10.
While waiting for product change over in the rock house, employee's left eye was hit by a piece of debris.
Upon investigation, other ee said he told them (did not report this to foreman). EE said he slipped coming across section, he told another ee he fell in a mud hole, about 2pm, told another ee he bruised his arm, that ee said go to MET, another ee asked to check his arm out he refused. 9/23 told another ee he fell in a mud hole.

Lifting large stone from ground, felt a ""pop"" in right shoulder with alot of pain. also injured right hand when stone fell and crushed hand between stones.
Employee was helping the welder holding a small piece of steel that was to be cut, in order to prevent it from falling abruptly, when the piece was cut the employee had difficulty and felt some bruising in his right hand, the hand appeared in good condition only a little swelling, later that day he felt pain in his shoulder.
Employee alleged pain in his back as he was unloading block. He finished his shift and did not report the alleged injury until a later date.

Employee reached into the roof bolter dust collector predump to check for blockage. He asked his coworker to check the suction. He thought he said turn on the suction, so he set the stab jack. This closed the predump door on the injured employees hand.
Employee was in the face of the #1 heading dusting. He was holding a bag of rock dust and throwing it on the rib, top and bottom; as he turned with the bag of dust in his hands, he felt pain in his lower back.
Ee was bending a 4' bolt in low top. Caused him to have a hernia. He is now off due to surgery.

On 9/21/2010 the employee was taken to the company clinic, was diagnosed with R-Elbow Inflammation and was returned to restricted duty. On 9/17/10 the employee reported that while attempting to remove a guard, he pulled a muscle in his R-Forearm and refused medical treatment at that time.

While using a 2"" water hose to remove sand accumulations under the transfer conveyer, employee apparently suffered an abdominal hernia.
Off-hwy end-dump truck veered off roadway onto the 8'+ shoulder, struck a buried drainage pipe, traveled up the steep slope adjacent to the shoulder, struck a boulder (approx 5' in dia.) & power pole, turned over onto its driver's side & came to rest on the side of the vehicle with the driver's side of the bed of the truck resting on the boulder.
After turning a right handed crosscut, EE was backing out of the 4RT cut when a piece of draw rock fell. The draw rock rode back along the rib. The rock was approx. 12' L X 3'-4' W X 6"" Thick. The rock pinned his right foot to the mine floor.

During operator pre-shift inspection operator added coolant to dozer. After operator added coolant the radiator cap was not installed on properly. Operator at 1:00 p.m. noticed cap was on wrong. Operator shut off engine and tried to loosen radiator cap which resulted in hot coolant to be released.
Employee States that he was picking up a tram motor for a little mac man trip, he felt a pain in his lower back.

Was shoveling along belt, rotating on knee, when he heard a pop. Knee was swollen when he got outside, he thought it would be ok, but was still bothering him on Monday when he finally saw doctor.
Survey crew was traveling to #1 section when they approached end of the track the mantrip slid into a ballast car. The injured EE riding in front of the mantrip had placed his hand across the back of the seat to brace himself. The batteries rocked forward catching the EE's hand between the batteries and the metal back of the seat.
EE reported this accident to me on 9/20/10 and he states that he was trying to help hook rail car to the mantrip. He states he asked the driver to back up because he could not get the hook and his left hand was on the hook has the mantrip operator reversed. EE states the hook smashed his finger.
Injured employee was putting the track tension cylinder back on the machine along with the track hoe operation. A chain was used to lower the cylinder in place. The chain was connected to the track hoe bucket. The injured ee had his left hand lying on the cylinder. His hand was caught between the bucket and cylinder.
Taking bolts off of the 4 foot piece of the steel set.
Miner was in scoop bucket feeding duster, scoop lost brakes and slid under belt line structure with him in bucket. Miner had cuts, bruises and contusions.
Employee had gone to put oil in water pump at washplant, had put in oil and when pump was started the discharge hose slung around and hit him in the chest knocking him down. Complained of rib pain.
EE was operating a CAT 325 Marsh Buggy and a hydraulic line broke. He got out of the cab to look and slipped on hydraulic fluid that was on the track. He hit his right knee on the cleat of the track.

When the tree fell, a grapevine that was unseen in the top of the tree pulled a dead tree approx 6-8"" in diameter down and struck EE, knocking him to the ground. He was seen on Monday Sept 13 and kept off work pending follow up on Sept 16. On Sept 16 he was released to full duty and told to attend physical therapy.
Employee thrown around when hot change bus hit berm. Cut to inside of lower lip and chin requiring stitches. Put off work pending follow-up on 09/13/10.

Employee was thrown around in bus when hot change bus hit berm. She suffered a broken nose and whiplash injuries to the neck. She was put off work pending surgical repair to the nose and subsequent recovery period.
The injured stated that he and another employee were cleaning the raw coal bin. The coal fell down, and the employee leaned back to catch his balance. He fell, striking his tailbone and lower back. He did not begin missing work until 9/16/2010.

He turned to his right to walk back to the face. A small hump in the floor caused employee to roll his right ankle causing the fracture to his right tibula and fibula.

EE was working in waterhole where mix had been spilled. He got his feet and ankles and lower legs wet causing burns.
Employee was assisting in removing liner bolts and the pipe was hit and the opposite end hit him in the abdomen, where he recently had surgery.
Stepping down from a manlift. EE said it just felt as if he pulled a muscle and was able shake it off. This was not reported to supervisor because he didn't realize the injury was that bad.
Coming down the stairs from pulverizer slipped and while grabbing the hand rail to catch himself from falling it pulled left shoulder.
While hanging mine curtain felt pain in left knee. Not sure if he twisted it or not. He was seen at Brodaus Hospital, he was examined and an x-ray was taken. All pins were still in place and the ACL surgery appeared to be fine. He returned to work without missing any scheduled days. Note; this injury occurred on 9/8/10 and became a reportable injury on 4/14/11.
While climbing onto the loader, my foot slipped on the second step causing me to loose my hand-hold and fall approx. 4-5 feet to the ground. This caused a strain to my back and a sprain to my wrist.

As the ee feet reached approx 7 feet on the fixed scaffold ladder, it appeared the ee missed his next step down with his left foot. Simultaneously, the ee lost his hand grip on the ladder and fell to ground.

Contractor employee under our suprevision was removing drill steel from hole and struck his left wrist on a metal burr, resulting in laceration. He was terminated for failing to follow company policy for seeking medical attention.

The injured was dragging a rock dust hose while rock dusting. Rock dust hose caught on something causing pain in his lower back. Has continued to work since 4/27/10. During this time he received PT. An examination shows mild arthritis of the right sacroiliac joint and degenerative disc disease in L%-S1. This is being reported as an Occ Ill do to repeated trauma to lower back.
Employee was in car loading from miner when a 6 inch by 8 inch rock hit him in the ribs. This rock slip off and landed on his ribs.
An employee was climbing an access ladder, had to step up in an awkward position and strained his knee. He was evaluated by a Physician and surgery was performed. It was made known on 11-3-10 that the repair was from an acute injury.

A rebar t-handle was being used to pull out a grease follow plate out of an old grease drum. When suction was relieved the handle came up and struck the employee in the eye.
EE was 1 of 2 man team using JBC powershot to clear lower diverter pipe choke--at initiation of the 7th shot, the shot tube was ejected from grip case with force followed by a projectile (scale) that ricocheted off structure back 16' to where EE was standing NW, 160 degrees from port opening-projectile struck EE in left side of neck.

Employee was riding on the outby end of a 12 man mantrip. The mantrip was traveling to #3 section when at break 24 on 3-b belt, the mantrip hit an 8 1/2"" piece of rail that was broken, causing mantrip to bounce. The employee hit the back of his head on the battery lid.
Employee was walking belt line when he fell and tried to catch himself and injured his right ring finger.

While cleaning out concrete footing with a backhoe the operator shifted the bucket to the left and boomed down. The edge of the bucket swung farther than expected and a tooth on the bucket struck the left foot of an employee that was directing the operator.

While cutting conveyor belting with a utility knife the knife slipped cutting ee left thumb, required 2 stitches.
EE was hanging a fly pad in the face of the section. He stepped in a hole and twisted his lower back. EE did not work on September 11, 2010. He did not work on September 13th, 14th 15th. He did work on September 16th, 17th, 18th, 19th & 20th. He did not again till October 4th, 5th, 6th. On Oct 6th he worked 6 1/2 hours. He has not worked since Oct 6th to date.
Section foreman was conducting pre-shift exam for 3rd shift, when a rock fell from between the bolts striking him on top of the head. EE started missing work on Monday September 6 from his injuries.
EE was getting out of truck cab to use bathroom and in the process of using bathroom, he let the truck door shut on his finger.

Employee was carrying a box of core samples and stepped off of a sidewalk. His ankle rolled causing a fall and injury.

Using a forklift, a large piece of steel plate was being flipped. The plate flipped opposite the way it was intended. When it fell, it hit the injured on the back of the heel and broke his lower leg.

A maintenance technician was in the process of loosening a hose on a pump. As the mechanic pulled on the wrench, the fitting came loose unexpectedly and the mechanic felt a pop and pain in his right shoulder. He was taken to his physician, given restricted duty til 10/08/10. On 10/08/10, he returned to the physician, restricted duty was extended pending an MRI. ONGOING.

EE was lifting heavy metal plates from a hammer-mill in a constricted location. This caused the lifting to be awkward and in various positions. Trauma was done to the trunk of the body, most likely resulting in a hernia. The doctor has not yet been consulted and no time has been missed as of this point; on self imposed lite duty restriction.
Employee was cutting steel plate 1/2""x40""X15' from a larger sheet, when the cut plate fell to the ground, then fell on employee.
Miner was changing a drill bit on a DX 800 track-mounted slope drill. While walking back to the pickup truck with the old bit, a loose rock slipped from under his left foot causing him to twist his left ankle. Miner report the injury and told his supervisor that he was OK. On 10/11/2010 miner informed me that he needed to go to the doctor.

The ee was adjusting a chain holding an electric motor for a belt drive, the scoop was elevated 10 to 12 inches off the bottom and when the ram was pushed out the scoop hydraulic ram tube came down catching the injured foot This 7000-1 is submitted to abate citation #8182155 This accident has been reported by the contractor MVK

Employee was standing on a ladder, removing a 32lb. box filled with bottles of deodorizer from a 6' shelf. The box fell on his arm causing a torn tendon to his right bicep. He is scheduled for surgery on September 23, 2010 and will be out of work.
Employee was uncoupling hitch from supply car. When the coupling pin was removed the hitch caught the employee's finger between the hitch and the supply car-resulting in a laceration to his index finger on his right hand.
Employee was begining maintenence on a crusher. He was lifting the top and it slipped from his hands and fell on his leg and foot.

While working on top of the water truck employee tripped and fell into the handrail. Injury was later diagnosed as torn rotator cuff. As of Nov. 1, 2010 employee is unable to perfrom job duties. Surgery is scheduled for Nov. 27, 2010.

Employee fell 4 ft from ladder when it shifted, while he was climbing it from shaft bottom back into the skip.
Employee was using a 3' line up bar to hold liners up through a bolt hole in feedcart. Another employee was using a forklift to push liners into place. One of the liners got wedged in the feedcart so the forklift operatior backed up to reposition. The liner slid down and hit the bar causing it to strike employee in the face.
Employee was operating a Cat 988G loader when a hydraulic line failed causing hydraulic fluid to go through the front window and out the back window - Striking the Employee in the chest and ribs.
EE was scaling roof on Section 003 6B beltline and track, when a piece of drawrock fell, hitting him on the right foot. The rock measured 4.5 inches thick and 43 inches in length by 19 inches wide.
Employee stated that he was helping to replace a screen on the #27 vibrator. He bent over to lift the screen out when he felt pain in his lower back area.
