#31 Hoist down- reset switch inoperable. Hoist down for 1 hour 15 minutes.

A roof fall was discovered on the slope track during the fire boss run. Fall measured approximately 12ft long, 12ft wide, and 7ft high. No injury occurred from this fall.

An unintetional roof fall occurred in the no. 7 entry two breaks inby spad no. 7154. The fall measured 75' long, 19.5' wide and 4' to 6' thick. The travel of men and airways were not effected.

A thunderstorm interrupted power to hoist. At 2:20 a.m. the power was restored, but there were problems resetting the hoist. At 2:40 a.m. MSHA was notified of the problem via the hot line. At 2:42 a.m. the electricians found that a fuse had blown and replaced it, bringing the hoist back into operation.

2 blocks inby spad #806 area had potted out some. While setting additional timbers around the area, more rocks came loose pulling bolts. Total area measured 20' W 20' L and 10' H approx. There were more timbers set and area posted and dangered off. Additional supports are being set around the block due to additional rock potting out.

An unintentional roof fall occurred at 60.5 CC of the 6th West Tailgate in the #1 entry. The fall was approx. 18 feet wide and approx. 10 feet thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up.

A material trip was beling lowered into the mine on the material slope. The trip stopped suddenly approximately 500 feet into the mine cause the safety slings attached to the cars to break allowing a car to travel down the material slope uncontrolled.

An unplanned roof fall occurred on #3 belt line in the #4 entry near spad 3476. The fall measured 25'L X 30'W X 6'H. The accidents were reported. This area was developed 01/16/2007.

A roof fall occurred at the #1 belt in the #3 entry at break # 25. The roof fall measured 18 feet wide, 50 feet long and 5 feet thick. Persons, equipment nor ventilation were effected. This area will not be rehabilitated.

A roof fall occurred in the underground magazine area between 7/29, Thursday, AM and 7/30, Friday, AM. It was discovered during the morning mine inspection on 7/30. A slab of rock approximately 3 inches thick and measuring roughly 10 ft x 10 ft fell from the roof onto a dry van used to store ANFO breaching the roof.

A roof fall was found in 8 main track entry at break 18. The fall was approx. 6'x24'x16' wide. No ventilation was affected, the fall was on top of the roof support arches that were in placed prior to this fall.

Roof fall was discovered about 4000' outby working section in #3 entry crosscut #34. The fall did not affect ventilation, impede persons or affect any equipment. Fall was discovered at 12:20 pm

The #1 elevator was out of service for a period of time exceeding 30 minutes, because the car would not return to the surface from the bottom. Schindler was on the site at the time and adjusted the regulator ""N"" contactor. The elevator was tested and placed back into service at 11:53 AM. No persons were on the elevator at the time.

An unplanned roof fall occurred in the #3 entry 40 feet outby spad #113 on the #5 beltline. There were no injuries or equipment involved. Fall area will be rehabilitated. Area was in with 4' resin bolts. 6 cribs had already been installed prior to fall. Ventilation and passage were not affected. State and Federal agencies were notified immediately.

A control switch needed replaced on the cooks branch elevator. The elevator was taken out of operation at 8:00 A.M. and was put back into service at 9:00 A.M.. No miners were working under ground during maintenance on the elevator.

An unplanned roof fall occured in the #3 entry near spad #107 along #5 belt. The fall measured 30' long, 20' wide and 5' thick. There was no equipment involved. Ventilation and passage of persons was not affected. The fall will be rehabiltated. The fall was breakered off and both agencies notified immediately.

A fall of roof occurred in the Main South Main at cross cut # 9, # 3 entry. The fall measured approximately 90 feet in length x 25 feet in width and the thickness was unknown. The area was supported with 6 foot fully grouted roof bolts.

A roof fall occurred above the anchorage zone on the Loaded Track of the Rotary Dump. The fall measured approximately 25' in length, 16' in width and 6' high.

During the preshift examination of the isolation stopping in the tailgate, a roof fall was discovered just inby XC-25 in the Number 1 Entry. The exact time of the fall is unknown. The area had previously been dangered off due to hazardous roof conditions. Air velocities on the LW face were negatively impacted.

The roof fall occurred inby the loading point on the C-6 Section (#5 Crosscut, #2 Heading). The area was bolted with 6' SuperTwist Bolts. No equipment or ventilation was affected by the fall. No injuries resulted.

An unplanned roof fall occurred approximately 3200' from the face in the #6 entry at spad #17766 approximately 50'x20'x6'. All entries were breakered off and the area will be rehabilitated.

A roof fall measuring 20' x 22' x 5' occurred in the No. 2 entry near spad No. 5203, MMU 004-0. 18 cross-cuts outby the face.

A roof fall was found at cross cut 113 entry 6 in 6RMS. Fall was in 4-way intersection that was mined in March of 2010. Area had bee supproted with 8' full resin bolts. No injury or property damage resulted. However, it did loosen an intake stopping. There are no plans to clean up this fall.

The production skip tripped to manual mode. It was restarted several times, but kept tripping out. A supervisor was called to the scene who decided that the skip would be down for a while. A mine evacuation was called at 2:45 pm. The responsible party for the hoist electrical system was called. Problem was fixed which allowed miners to return to work at 3:35 pm.

A roof fall was discovered in the old works on the Big Buck Mains, #3 entry, old #3 head, brk #1. The fall measured approx. 40' long x 18' wide x 6 to 7' high. No injuries resulted from this fall.

An unintentional roof fall occurred in entry #3 between the 49+80 intersection and the 50+55 intersection in the first east panel off of the first north submain. The fall measured 25' x 18' x 4' high. It did not affect ventilation, passage of men or cause personal injury. It had been bolted with 4 foot fully grouted roof bolts.

Miners went to board the service cage to exit the mine. They noticed the man door was missing at 9:00 am. Called a mine evacuation at 9:05 am. The door was repaired and the shaft was inspected. The hoist was operational at 11:15 am and miners returned to work.

A roof fall has occurred in the intake airway 60 feet inby spad no. 110 in the #3 entry. The fall measured approximately 5' thick, 20' long and 20' wide. Neither persons ventilation or equipment were involved. This area will not be rehabilitated.

4701 Hoist had a bridge rectifier failure. Electricians replaced rectifier fixing electrical problem with Hoist. No one was on skip when failure happened, employees were getting ready to go underground to work on leaky feeder system.

A roof fall was found at 1:15pm at entry 1 xc 238 main south. Fall was in a four way intersection that had been supported with 4 & 5' full resin bolts. In addition, truss bolts had been installed in this intersection. No injury or property damage resulted. At present time there is no plan to clean up this fall.

An unplaned roof fall was discovered by the mine examiner on 07/27/2010. The fallwas in the out by intake at spad 1615 in #5 entry. The fall measured approximately 30' long by 19' wide by 8' high.

A grass fire occurred in Sandow Mine on Tuesday, July 27, 2010. The fire started at approximately 2:30 p.m. on buffer property owned by Alcoa Inc. east of the mine reclaim area. An electrical spark started the grass fire. The Alcoa Rockdale and Milano volunteer fire department responded. No injuries resulted from the fire and it was under control by approximately 3:30 p.m.

The hoistman reported having electrical problems with the hoist. Electricians were called out to diagnose and remedy the issue. After 30 minutes MSHA was notified via the hot line and the crew exited the mine through the secondary shaft.

EE was in the elevator coming to the surface when the elevator stopped approx. 5' from the top. The EE was safely removed from the elevator at 4:35 a.m. by Schindler elevator service. The problem was a bad contactor.

While dewatering/rehabilitating the Black Oak Mine, the North Mains and East Mains Intersection was pumped down exposing a roof fall located near spad #1475. The fall is 1 break outby the butt-off of the North Mains and is approximatley 5' high in the intersection.

A 7X16X2.25 deteriorated section of the roof strata became dislodged around the permanent roof bolts and fell on the manbus trackway. The area had been bolted in the mid 90's using 6 foot conventional bolts.

A roof fall was discovered on the C-6 Section (#4 Crosscut, between #13 and #14 room). The fall occurred 450' outby the feeder. The area was bolted with 6' and 8' SuperTwist bolts and 12' cable bolts. No ventilation, travelways, or equipment were effected. No injuries resulted from this fall.

The mine encountered an unintentional roof fall on the Main South, Entry #4 (belt), between XC#62 and #63. The fall measured 40'L X 20'W X avg. 5' thick. The area was developed in April of 2000 and supported with 4' fully grouted bolts. Supplemental support consisted of 6' fully grouted bolts. The fall will be cleaned as per approved site specific plan.

An unintentional fall of the mine roof was discovered at #2 crosscut in #3 entry in the Cedar Grove. The fall was approximately 6 to 7 feet high, 20 feet long and 20 feet wide. The fall was immediately reported by phone.

A fall of the roof occurred in the SE Mains at crosscut 186 in the #3 entry on the supply road. The Measurements of the fall are 35 ft. in length X 20 ft. wide and 6 ft. high. Ventilation, passage nor personnel were affected.

An unintentional roof fall occurred at the Galatia Mine New Future Portal 6 Seam Submain Return Entry #1, 275.45N, Crosscut #4. The fall was approx. 23' long, approx. 28' wide and approx. 8' thick. The fall will be cleaned up. Fall #27L.

A west drive communication fault caused the hoist to kick out. Normal reset procedures would not allow resumption of operation. Component was replaced and hoist returned to operational status.

Regular maintenance was being performed on elevator. While changing magnetic switch, it was found to be a stonger field than original. Replaced with correct switch, which took longer than expected. Returned to full service at 5:30pm.

An unintentional roof fall occurred at the Galatia Mine New Era Portal 6th West Headgate, MMU 059-0, in the #2 Entry, approx. at Tag #8725. The fall was approx. 15' long, approx. 15' wide, and approx. 9' thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up. Fall #27 K.

Security guard made rounds and saw that a 992G CAT Loader was on fire. This was a late Sunday night/early Monday morning, no employees were present.

A roof fall occurred at 4-5 entry on the mantrip entries at the #1 portal. The fall of stone was 12 feet wide, 18 feet long and 5 feet high. The fall was not above the anchorage and occurred on the track entry. Area mined 1970.

Examiner was starting pre-shift, was checking fan & saw no water coming out of mine. He went to investigate & found roof fal in #2 entry of 1st NW Mains, 120' inby station #116. This is about 1500' inside mine. Fall was in intersection about 4 1/2' to 5' high, 25' by 20' Long & 18' to 20' wide. Area was bolted with 4' resin bolts. Area was dangered off on all approches with cribs

While the midnight mine foreman was exiting the mine he encountered an unplanned roof fall. It was located in the secondary escapeway for the # 3 section. Spad # 3101. All approaches were cribbed and dangered off, also the lifeline was re-routed so to travel the escapeway in it's entirety. MSHA was called at 06:25 a.m the same day. An inspector looked at it on the Monday the 26th.

A roof fall was found on 7/24/10 at approx 3:00 pm in the #6 intake entry approx 100' outby SS 2197. This was a continuation of a previous roof fall. The fall did not cause any injuries & did not affect ventilation; and did not cause entrapment. The fall will not be cleaned up and the area was breakered off.

The main transformer that operates the Production Hoist blew. The Production Hoist and Shaft are the second escapeway from the mine. The mine was evacuated. A new transformer was installed and the Production Hoist was placed back in operation.

An unplanned roof fall occurred in the main line intake, one crosscut inby spad # 88, in the # 5 entry. The fall measured 19'Long X 19'Wide X 8' in Height. No accidents were reported. This area was developed in May 2008.

While operating a face drill, the operator noticed flames behind her in the articulation area of the truck. The operator deployed the fire suppression system, which did not completely extingush the fire. Operator called for help.

The Hitachi Scaler caught fire at 10:50am. A fire extinguisher was used and was ineffective. The mine was evacuated. The fire burned itself out at 2:30pm.

The mentioned mine has experienced a non-injury un-intentional roof fall outby the 002 working section located between #7 and #8 entries approximately 15 cross-cuts outby the working section.The fall is 20' wide, 25' long and 6' high with laminated sandston and shale. The are was initially supported with 60"" 3/4 fully grouted rods.

Roof Fall - Intake slope entry, starting at the outby side of Break #5, approximately 28 feet long, 17 feet wide and 5 feet in height. No-one was injured and no equipment involved. Found at 7:37 am and reported to MSHA at 7:43 am. MSHA inspectors on site after reporting by phone.

The elevator at the camp creelportal was down for more than 30 minutes due to a ground in the power leads coming from the substation to the main building. Power was fixed going to the building. Persons could travel via the slope by slope car or walk.

The weekly examiner was making his weekly examination when he found a roof fall in the outby area of the mine. The roof fall is located at survey station # 6829 in the #5 entry at crosscut #24 of the north mains. The roof fall is approximately 6 - 8 feet thick, 20 feet wide x 26 feet long. No injuries or equipment involved at the roof fall area.

On 7/17/2010 an unplanned roof fall occurred on MMU 002 in #6 entry, cross cut #4 in the 1B Submain off the 4th S.E. Sub-Main. The fall was approximately 25' long X 17' wide X 8' thick. It was a non injury accident, and the fall will not be cleaned up. The area was bolted with 8' and 10' roof bolts.

Operator was on mixing plattform in specialty plant when he heard a couple of pops in the corner of building. He went out of building and seen 5-6 foot flames coming from exaust fan in compressor room. Called foreman which made assessment imediately to call 911 to get fire department on its way. There was moderate damage to compressor room and batch room of specialty plant.

An unexpected roof fall occurred #3 entry, x-cut 73 in our M-4 Mains. The fall is adjacent to our established secondary escapeway. The fall did not cause personal injury, impede passage of miners or disrupt ventilation. It measured app. 18' X 20' X 8' high.

A roof fall, 40' x 18' x 6' thick occured in 4 Lt. Main near spad 4434 approximately 8900' from active face. No travelway or airway was effected by the fall.

An unintentional roof fall occurred at the number 11xc of the 2nd Main East in the number #3 entry. The fall was approximately 35 feet long and 8.5 feet high and 17.5 feet wide. The fall was in a area that had floor to roof support installed to narrow the entry down. Area had shown alot of water from the top.

Had fall at brk 14 spad #77 in #2 return entry. Fall was in area of laminated top. Additional cribs were installed and complete area was dangered off. Fall was approx 17' W x 20' L x 8' H. Fall was discovered during weekly run.

Seismic event was initiated at blasting time occurred in the vicinity of 4900-310 Stope. During swing shift, the mine was inspected and damage was found. Upon determining the extent of damage, MSHA was notified at 1-800-746-1553 hotline.

Employee was unloading a load of crusher run into the storage pile and failed to fully lower the bed of the A40 Volvo truck. He drove under the belt of the rock crusher and hung it with the bed of his truck and destroyed it. It was pulled from the building and bent and is no longer operational.

An unintentional roof fall has occurred at the #7xc of the 2nd ME in the #4 entry. The fall was approximately 30 feet long and nine feet high and 18 feet wide.

Roof fall was found at 9:10pm in the return on 1 left panel in 20 south mains at break # 41. Fall started in the intersection of #7 entry and went approximately 150' thru the intersection in # 9 entry from spad 2245 to spad 1166. This area was mined around the first of June. Area was dangered off.

CM was in #1 entry, mined into source of water, an ungrouted core hole, unknown driller. Elevation changed 3 brks (180 ft) outby face. Water accumulated in #1 & #2. Moved equipment, began pumping. The water did not present any danger to the men. We do not believe this is a reportable inundation.

A fuse blew on the drive for the number one motor on the Production Hoist. The Production Shaft is the second escapeway from the mine. The mine was evacuated. The fuse was replaced and the hoist was placed back in operation, and employees returned to work.

A roof fall appx. 20'x60'x4.5' occurred in the #1 entry, which is the return entry, of the 002 section at spad # 47072 at 7:30 PM. No equipment or persons were involved.

A roof fall was discovered in the 3rd left return off #2 mains at spad 63665. The fall was 2000' outby the working section, neither ventilation nor the passage of men was not affected. The fall measured 8' high, 100' wide, 175' long. The area was developed Feb 2010 with 48"" glue bolts as primary roof support. The entrances to the fall was timbered.

Two 4 foot split set bolts fell out of the back as a result of blasting to advance the face. No one was in the heading at the time of the fall. It was discovered by the next shift.

Roof fall located on the number 3 belt line 1 crosscut inby the head drive at spad number 41. The fall was 18 feet wide, 20 feet in length and 9 feet high

While the examiner was running the Rt Return he found a fall 70' long in 0 Entry of South Line Room 3 XCut Outby Station # 8533. The Fall was above the bolts. The area had been bolted with 6.5 3 piece bolts with T-3 Channel and 10' Cable Bolts. Both agencies were notified, the Fall is in ouby area and did not impede travel and/or air.

Two miners were trapped for 1 hour and 20 minutes on the Miracle Run elevator due to a communication cable intangling with the slow down limit cam.

The travel road was re-routed and the fall has been secured from all sides. All agencies were notified and MSHA has released the area. No one was injured in the roof fall.

#1 Elevator was out of service for a period of time exceeding 30 minutes due to a bad MG set regulator. The MG set regulator was changed and the elevator placed back in service on 7/6/2010 at 8:30 pm. The mine was on vacation at the time. No escapeways were affected and no individuals were on the elevator at the time.

At around 9:30 pm, a phone call was made to inform OhioAmerican Energy that a small fire could be seen at the Prep Plant. The mine superintendent and safety director then drove to the Prep Plant. Upon arrival, a small fire (approximately 6 feet x 6 feet) could be seen burning on the coal stockpile. A D11 dozer was utilized to spread out the coal and quickly put out the fire.

He was taking a rub rail off of the miner and caught his right middle finger between the rub rail and lolo structure. Finger was amputated to the 1st joint.

The other EE then pushed the boring bar through before his hand was clear, pinching his ring finger between the bar and the bore of the bearing, cutting off the end of his finger between the fingernail and knuckle.

Man was watching bolt man pin up spot bolt, when a piece of rock fell striking machine. Man had his hand on side of machine where rock landed, rock was 2'X1'X8"" thick. Rock landed on his left hand catching and cutting his ring finger.

Employee was dismounting a front end loader. As he was descending the stairs his ankle turned and he fell down the stairs to the lower platform. During the fall he landed on top of his left leg. Injured employee was in the process of removing the bottom section tub from a belt drive gear. The tub fell and pinched off the end of his left thumb. He had surgery to repair his thumb.

EE had gone up into the cab of the Cat 988H loader he operates approx. 15 min prior to shift. The EE caught his heel on the steps while descending from the cab & fell striking the right temporal area of his head on a 1"" diameter steel rod at the rear of the machine. The rod is used to secure wheel chocks during normal operation. He received multiple injuries to his skull.

EE was loading conveyor belt on a belt car and was using clamp (rope dog) to pull on the belt. The clamp slipped off the belt, causing an injury to his left shoulder. Possible rotary cup damage to shoulder. He started missing work from this incident 09/07/2010.

He refused medical attention. 2 others helped with hanging. Went to Dr. and referred to orthopedic. Diagnosis, back strain on 8/17/10. He is seeing a chiropractor.

EE turned switch off on belt line and crossed belt line. EE set spads and then attempted to cross over belt line again. The belt line began running and EE fell off the belt line and hit his back against the rib. This caused an injury to his lower back.

While removing one box out of a taller box, EE felt pain in his right groin area. He went to a physician and was removed from work for an inguinal hernia.

EE used torches to cut ears off shaft when roller grease ignited in his face. He was transported to the hospital with 1st degree burn to face & difficulty breathing. He was intubated & sent for more testing at a specialized burn unit.

While disconnecting the positive terminal on the battery, the wrench shorted across the ground causing the battery to explode.

Employee was unhooking duster from mini-trac when he felt a pain in his shoulder. He told several co-workers about this. This was on 7/28/10. Employee continued to work the rest of the week. On Monday, his back and shoulder were hurting so he went to the doctor on 8/2/10. He was given muscle relaxers and 2 days light duty.

The operator got out of the 992G loader to move some wet hole bags from the spoil pile when a rock rolled down the spoil and struck him in the calf of the leg. This incident happened about 8:30 PM. He reported it to the foreman about 12:00. He refused medical attention and transport to the hospital. He went to the hospital after his shift.

While dumping a load of coal, the truck was leaning too much to the passenger side resulting in both truck & trailer to overturn onto the passenger side.

Miner was walking on the catwalk of the main screen plant when he contacted an electrical line, (which had some insulation rubbed off due to placing a screen up against the line) the line arced on the hand rail and shocked the miner, causing him to fall against the hand rail straining his back.

EE had just replaced a drum of grease on his Lube truck and he screwed his grease hose onto it. When he shifted the lever to charge it with air, the end blew off and hit him in the chin. He received stitches.

EE was lifting a piece of channel iron to be installed on a shop wall. He was about half way up a six ft. step ladder. He lost his balance. He stepped backward down to the ground to regain his footing, but, the piece of channel he was holding remained on the top rung of the ladder. His chin struck the iron on his way down, resulting in a laceration and two cracked teeth.

As he was pulling on the hose his feet slipped out from under him and he felt a burning sensation in his groin area. The man reported pain in his right lumbar and right knee. The man reported off on 7-29-10 with a non work related medical problem. On 8-4-10 he completed a work incident report for his knee and back regarding a stated 7-27-10 incident. On 8-16-10 a medical examination indicated that the lumbar and knee pain were work related.

Employee was moving a miner cable to the side of the rib and ran into a bent roof bolt plate on the corner of a ledge.

EE was walking main north belt when a piece of coal/rock fell from the roof, striking EE on the left side of the hard hat. EE experienced stiffness to the neck and pain.

Employee was moving rock knockers to be placed on a forklift. One of them slipped and mashed & cut his left index finger requiring stitches.

The man was walking through the section when he slipped and fell forward. He attempted to break his fall with his arms and jammed his arms into the bottom. The man continued to work regular duty until 09/07/2010 when he reported off work.

Attempting to knock a pin out of spring board of miner with a drift punch. Believes that a piece of punch broke off and became lodged in his left forearm. This left a puncture wound and surgery was required to remove the piece of metal that required 7 sutures to close wound. Punch did not have a mushroomed head.

Employee stated he was lowering the canopy of the Bolting machine and had positioned his hand on top of the canopy.A rock fell from the roof and struck him on the hand/fingers. He was taked for medical evaluation.

Employee was assisting on blast crew as a driver & laborer. He was attempting to exit the rear of a straight van truck. He was using the rear door and door frame as hand holds & was reaching with foot to use ICC bumper as a step when his foot slipped. He fell to the ground injuring his right knee dan right shoulder.

Employee was walking beside the section feeder when he stepped over some coal and rock, the employee slipped and bumped his knee on a rock. Employee continued working until he was seen by a physician on 8/4/2010, at that point he was placed off of work and is awaiting surgery for his left knee.

Employee was operating a lo-trac scooping gob from a crosscut prior to the incident. While there were no witnesses and the employee cannot recall what happened, we believe his head came in contact with the roof or a 4 inch plastic water line.

Employee was shoveling out the hopper above #1 conveyor and struck his hand against the side of the metal hopper. Employee had a laceration on his right hand, middle finger that required medical treatment.

Employee tugged on the air hose he was using and fell back catching himself on the hopper.

Employee was attempting to clear a chute plugged with lime. Using a rod to clear the plug he struck a pocket of hot lime which blew back into the employees face entering his eyes behind the safety glasses he was wearing resulting in the use of antibiotic medication.

The injured employee was walking from entry #4 to entry #6 to unlock a piece of equipment. When crossing entry # 5, inby the feeder tailshaft, the corner of the inby rib fell out striking the employee in the face. The coal rib measuring 2'x2'x2' pushed the employee to the ground. He had surgery on 9/10/2010 for a deviated septum, which took him off work.

The mine operator was made aware of the employees hospitalization on 7-12-10.

The rock measured 10'L x 3'W x 0-11"" thick, and fell from a height of 7'11"".

Hydraulic wrench slipped and hit the right lateral side of his calf and caused pain to the right medial side of his knee. EE was examined and returned to full duty, however, in a follow up exam and MRI shown there was a torn ACL, MCL and mincus. He was put on modified duty on August 2, 2010.

Employee states he fell while getting off of a loader. He alleges he mis-stepped near the last step of the loader & fell to the ground. He tried to catch himself with his arm & fell on it as well. He originally thought the fall was not serious. He had continued pain and saw a doctor that put him on modified dutyuntil Oct 5th. The Dr. susequently took EE off of work.

Mud pump had lost its prime, operator shut it off and took off prime cap to fill. The cap had pressure and hot mud splash on face and neck, minor burns on face and neck.

The employee was disconnecting the road widener machine from the front of the loader boom. The left coupling pin got stuck and he was using a hammer and a large bolt to knock the pin loose. Employee was getting ready to pick up 200 lb shaft with crane when another contractor pulled on sling to tighten it. While employee was looking up at the hook being lowered, shaft rolled off stand and landed on right foot behind steel toe. Other contractor didn't speak up to alert employee.

Concrete floor was sloped and wet but clean and EE was wearing rubber boots.

Pushed a shovel adapter to better position so hook up to boom truck would be possible. As I pushed on the adapter I noticed a sharp pain in my left testicle area. Pain increased. Came back to shop and reported to my supervisor, saw EMT. Went to hospital to see doctor.

Employee was walking around heat exchange floor. While he was walking he twisted his ankle. He was sent to the doctor and was diagnosed with a left ankle sprain. He returned to work the same day on light duty.

Driller was adding drill rod. Top of rod contacted sheav wheel and dropped back on hand. Finger pinched against feed frame of drill.

Injured was loading kennedy panels in the scoop bucket when his left hand was smashed between the scoop bucket and the panel. Diagnosis: Fractured hand. Injured has been placed on restricted duty.

Fall from machine

Employee was stepping down off a piece of equipment and slipped off the step and fell to the ground landing on his foot dislocating his ankle. The force of the dislocation tore the skin from the top of his leg around to the back requiring stitches to close.

Three employees were positioning a 1""x8""x8' piece of plate when one end slipped off one stand and pinched the right thumb between the plate and the skid they were working on. Right thumb was fractured.

While replacing parts, the person who was injured was holding up a cylinder frame. The cylinder was only connected at the base of the frame and when it was lifted, rotated down injuring a finger.

Employee was carrying trash through a repair bay doorway and was watching equipment movement in front of the building. Employee walked forward stepping on the access ramp edge. Employee fell.

EE was using a sledge hammer to tighten down nut on crusher mantle liner. He struck his right thumb nailbed with the hammer.

The injured employee finished guiding a reel of ground wire onto the back of truck. The injured employee descended down from the bed of the truck down to the first step (driver's side). The employee felt a sharp pain in his left knee when he stepped down with his left foot to the ground.

Wheel chock fell from top of truck and caught employee's ring finger between the chock and the edge of the truck.

EE was moving a ladder stand when the eliminator fell and he tried to catch it. The eliminator hit his right middle finger & smashed it between the eliminator and the stand. His finger was x-ray'd at the local hospital and revealed a fracture and was given pain medication (non-prescription). He returned to work with no restrictions the same day.

Employee cleaning in the lower section of the 18 D/L in the #3 Swing Compartment around 11:30 am. The employee was mopping in a front to back motion facing the #3 swing shaft and standing on the floor man-way cover. While mopping up some grease, employee's right foot slipped off the man-way cover rolling her right ankle and straining her right knee.

While lifting and lowering an outrigger extention block and pad on the condor 100 lift truck, employee states he mashed his right hand between the extension block and water cooler mounting bracket as he was lowering the extension block to ground level.

Employee states that while working tripping out a hole, the pipe wrench he was using slipped and snapped back hitting his left open palm.

Employee failed to report the injury to employer until July 26, 2010. Employee was on the third deck of the mill performing clean up operations. He had over filled the wheel barrow causing materials to fall on the deck. When he went to move the wheel barrow he slipped on the spilled material that he had dropped. He did a split and got up and continued to work.

Bending over to lift empty boxes felt sharp pain in lower back. Diagnosis hip flexor muscle strain. Light duty.

Employee parked the grease truck and was exiting the vehicle when his foot slipped off the step, causing him to fall to the ground, bruising his right lower back and right hip.

While attempting to remove the rear swing motor pedestial, EE pinched his left index finger between the pedestal and the right gantry leg. This resulted in a fracture and laceration to the finger.

Employee was cutting conveyor to size for installation using straight edge and utility knife. Utility knife slipped off straight edge and cut employee along right index finger of ungloved hand. Cut required 9 stitches to close.
