One of the jacks on the rope gripper was seeping oil and bled off until hitting a limit switch, shutting the elevator off. No one injured or trapped as a result of this incident.

Roof fall occurred at the #1 portal #3 track at the 5 X-cut. Fall was 40' L. X 17' W. X 10' H.
The elevator at the camp creek portal was down for more than 30 minutes due to a stop switch at the top level was bad. The switch was repaired and elevator put back in service. Person could travel via the slope be slope car or walk.
Roof fall in area that was timbered off and dangered off.
This was a unintentional roof fall in a X-cut between the travelway and left return entries. We had to stop production, call MSHA and a 103 K issued. MSHA then modified it to timber off all approaches and another modification to re-route the lifeline and build stopping to re-route the travelway. After completing 5 phases 0of a plan ,returned to normal activities
Mining #1 to #2 entry in 4 North, mine section, MMU-001, #15 x-cut. Water started coming out of mine floor, backed miner outby. Later water flow increased, shut power off in section. Water flow increased filling outby crosscut in the lower return entry. Ventilation had to be changed to ventilate section and pump water.
A roof fall occurred in the intersection of 2nd NW mains at spad #1012 measured approx 40'long - 19'wide - 10'high area was bolted with 6"" fully grouted belts and 10' cable bolts.

A roof fall was discovered on 10 belt at 10-38. Dimensions measure approximately 16' wide X 60' long X 15' high.
At 9:50 AM on 10/27/10, an unintentional roof fall was discovered during a pre-shift examination while an MSHA inspection was on-going. The fall occurred at Spad #165 on the belt. It measured approximately 25' L x 20' W x 6' H.
Fall was found off of #2 south mains inby spad # 1024. Fall was approx. 25' wide x 20' long x 6' to 7' high.
Four contractors with Safway Scaffold were stuck in the K3 elevator. Executive Elevator was called to get them out.
Ray Jones Trucking who is a contractor CDA was dumping a load of coal from one of their coal trucks on the coal yard when the truck bed started leaning over and fell over on its side. No one was injured.
a roof fall was discovered around 14 break on 7 belt approx 6' high 60' long in #2-#1 intersection over into #1 entry.
A ROOF FALL OCCURRED IN THE 7TH SOUTHEAST MAINS RETURN AIR COURSE FOR THE 004-0 MMU AT SPAD NO. 3971. THE ROOF FALL STARTS IN THE 7 RIGHT CROSSCUT AND CROSSES THROUGH THE NO. 8 ENTRY AND STOPPED 25' INTO THE NO. 8 CROSSCUT. THE FALL IS APPOXIMATELY 6'-8' THICK, 19' WIDE, AND 70' LONG.
17W Section, #3 Entry CBM Well BH-2A (please note that this 7000-1 form is being filed for reporting compliance purposes only, as we do not feel that an inundation occurred). BH-2A was intersected and the approved plan was followed. The hole was grouted to plan and mining resumed.
Had a continuation of previous fall in #4 entry brk #7 spad #28 whick took out intake stopping. Area was supported with jacks and cribs and stopping was rebuilt. Est fall 5'H-10'W-16'L.
Roof fall found during weekly exam. Found in an autby area of the right return approximately 1000 ft from the working section. 1 break inby spad #1048. The fall does not affect travel or ventilation. The area has been hangered off and supported.
An unintentional roof fall has occurred at the #17xc of the 2nd Main South between the #4 and #5 entries. The fall was approximately 60 feet in length and 18 feet wide and 8-10 feet high. The fall did take out a ventilation control and all work inby was ceased.
A roof fall occured in the #9 seam, 1st West Sub-Mains between #4 and #5 entries at crosscut #7. The fall measures approximately 50'x18'x10'-12'. The areas was supported with 6' tension rebar roof bolts. An undetected slip was determined to be the casue of the fall.
At an inactive area of the mine, waste coal spontaneously combusted near discarded haul truck tires causing the tires to catch on fire. The wind was blowing at 40 MPH. The fire was extinguished at 10:36 AM.
At 5:11 pm on Monday, October 25th the Kiln Operator received an alarm that the west explosion vent door opened. It apprears that a backpressure/ignition resulted from the waste(excess) air valve to the firing hood being completely closed.
The mine encountered an unintentional roof fall on the Main South, XC68, between entries #2 and #3. The fall measured 17'W X 15""L X 4' thick. The area was bolted with 4' fully grouted bolts in May of 2000. This fall will not be cleaned up.
An unplanned roof fall has occurred in the No. 3 netural entry two crosscuts inby survey spad No. 3127, the fall measures 20 feet long, 20 feet wide, and 5 feet thick. The fall was above installed roof bolts. The fall does not impede travel or affect ventilation. All areas of the fall will be timbered off and danger signs posted. The fall was reported to MSHA.
A fall of the roof occurred in the South East Life of Mine C at crosscut 538 in the #3 entry. The measurements are 35 ft. in length, 20 ft. in width and 5 to 7 ft. No equipment, personnel nor ventilation was affected.
An unintentional roof fall occurred at the Galatia mine new future portal 1W HG MMU-005 outby crosscut 12 1577W #1 entry. The fall was approximately 20 feet long, approximately 18 feet wide and approximately 10 feet thick. The fall did not affect ventilation or passage of men. The fall will not be cleaned up. Fall #27A
A fall of roof occurred in the 2nd South East Submain at cross cut # 31, # 3 entry. The fall measured approximately 25 feet in length x 18 feet wide and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
Two employees were stuck in the K2 elevator between the second and third floors.
Roof fall discovered at 75 Brk #3 Intersection at St Zero return aircourse, blocking travel of weekly examiner.
Roof fall was found on 1 west mains 20 break, #6 entry in the intake. spad # 355. Fall was apppox. 25' long x 19' wide x 5' thick. Roof control was 5' torque tension bolts.
An unplanned roof fall occurred in the #2 entry of the old 4-A panel return between & to the right of spads 3615 to 3633. The fall did not block an escapeway, travelway or airway. There were no injuries or equipment damage as a result of the fall. There are no plans to clean up the fall however all approaches around & leading to the fall will be dangered & timbered off.
Unplanned roof fall in return on 1st left panel off of #8 belt in #8 entry. Fell from #10 Brk to 11 1/2 Brk (134' long X 8-10' high X 18-20' wide) between spad #2373 & 2384. No injuries, no equipment involved, no ventilation affected, no travelway affected. Date of development 6-29-2010.
At the M20 beltline #30 cross cut a fall of ground occurred. The fall was approximately 20' X 20' X 7'.
Damage was done to the hoist sheave wheel for hoisting men and supplies out of the slope opening. The lead car failed to stop before hitting hoist sheave wheel and damaging hoist cable. The cause was out of adjustment drive chain controlling the upper limit switch and the overwind switch was not positioned to activate.
Three unintentional roof falls were discovered during an MSHA inspection. The first occurred at the #5 entry at spad #83 (measuring approximately 6' H x 20' W x 50'L).
An unintentional roof fall was discovered during an MSHA inspection. (This was the second of three roof falls originally reported on 10/22/10). The fall occurred between #4 and #5 entries at Spad #111, measuring approximately 6' H x 20' W x 60'L.

A Rock Fall was discovered in #2 Entry #14 x-cut. Fall was approx. 20' X 25' X 7'H. There were no injuries, and ventilation was not interupted. There are several more entries available to make this weekly exam.
An unplanned roof fall occurred in the #3 entry on the #2 belt @ spad 0567. Fall measured 25' long x 25' wide x 8' thick. No equipment or injuries, area was developed 6/97. Roof fall did not affect ventilation or passage of persons. The fall will not be rehabilitated. Roof support consisted of 48"" resin. State & Federal agencies were notified immediately.

A fall measuring 35 ft x 18 ft x 6 ft occurred in the 1st North Mains one break inby spad 5415.
Fall was located on 1 west mains 9 break #9 entry. Fall was 20'W x 20'L x 5' Thick.
Citation No. 8497783 has been issued on November 1, 2010 for failing to complete a 7000-1 form for two alleged roof falls in the 1st North East Main in the No. 9 entry at spad No. 83+20 and 84+80. River View is submitting this 7000-1 under protest to abate the citation and we do not believe the alleged roof falls were reportable.
During a weekly examination an examiner found an unplanned roof fall in our M4 Mains. The fall was found in #1 entry, x-cut 52 through x-cut 54. It measured app. 120' long, 18' wide, and 15' high. The roof fall did not cause personal injury to miners, affect ventilation, or impeed passage of miners.

A roof fall occured in the 2nd West Main, #4 entry between spad 4+00 and 3+20. The fall is approximately 25 feet long, 19 feet wide and 7 feet high.
The #8 elevator was out of service for a period of time exceeding 30 minutes due to the inside door being ajar. No individuals were on the elevator at the time. All agencies were notified according to law. The door was closed, the elevator was examined and the car was placed back into service @ 1:35AM.
An unintentional roof fall occurred inby the dumping point on the 8Q longwall. The fall measure approximately 20'L x 23'W x 12' H.
A fall of the roof has occurred between the #7 and #8 entry of the 3rd SE mains two breaks inby survey station #2827. The fall is approximately 8' thick, 19' wide, and 50' long. The fall was found by company personnel and an OMSL inspector during routine inspections.
A Fireboss found the elevator doors at the Somerset Portal elevator open on the Surface landing. When he tried closing them they would not completely close which kept the elevator from operating. There were no persons injured or equipment damaged as a result of this incident.
An unintentional roof fall has occurred at the 14xc of the 2nd Main South between the #3 and #2 entry. The fall was approximately 25 feet long and 18.5 feet wide and approximately 7 feet to 18 feet high. The fall was approxiamtely 2 to 3 feet above the anchor zone and consisted of stack rock.
There was fire inside the Pier 6 switchgear room on #1 kiln at 12:36 on 10/18/10 that took down the #1 kiln. The turbosonic compressor breaker caught on fire and tripped the main feed in the sub at Pier 4. The smoke detector in Pier 6 switchgear activated and gave the location of the fire in the control room.
The Kuhntown elevator went down at 7:45am at the top of the shaft. There were no persons on board. The door switch was broken and replaced by schindler at 9:30am.
While the elevator was being reprogrammed from indians (online) it malfunctioned with an fault. MSHA was called @ 10:30 am and the state @10:42. Nobody was trapped or injured during the incident and both agencies were notified when it was back operational.
An unintentional roof fall occurred in the old 9E to 9F recovery #1 entry at the #1 crosscut. The fall measured approximately 18'W x 20'L x 10'H.
A small fire was discovered inside the surface maintenance office by security personnel at approx 10:30am. The mine was idle & no employees were present at the time of the fire. The fire was contained to this building only. The local Fire Dept was notified & upon arrival the fire was extinguished in approx 10 min. There were no injuries or fatalities associated with this fire.
New contract EE being trained on dozer on the raw coal stock pile by supervisor. After several hours, supervisor thought he was ok to work alone. EE pushing coal into large void and trammed too close to edge of angle, slide sideways and could not get out. EE pushed STOP control/all activites were shut down. A dozer brought to fill void, allowing stuck dozer to tram free.
At 8:51am at the G-8 Pump Power center (SN- 48207-7/05)located at crosscut #7 on G-8, an employee saw arcing and smoke coming from around the transformer inside the power center. At 9:11 am the small fire was out.
An unintentional roof fall occurred on 10-16-10. The fall occurred on #8 belt, #8 entry, left return at pad #3180. No injuries occurred from the fall (length 40' X width 19' X thickness 4'-6'). Support 4' fully grouted bolts.
A roof fall 120' x 20' x 6' thick occurred 200' inby spad # 4326 in the bleeder system. The area was bolted with 4' fully grouted roof bolts. The area is taking pressure due to undermining.

Unintentional roof fall that occurred at break 159 on the HG #4. The fall measured 8'hx20'wx40'l.
Intermittent brake problems. Cleaned electric brake coil and rod. Re-adjusted brakes. The #6 elevator was down intermittently from 11:55 pm until 3:30 am.
An unintentional roof fall occurred on the 1st South sub-main off the east submain in the #4 entry return, at 33+20. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 30'x25'x8'. It will not be cleaned up. It had been roof bolted with 6'fully grouted roof bolts.
An unintential fall occurred in the #7 2nd West Main entry at the 8+40'. The fall was approximately 15'X 15'X 8' on each side of the crib setting on the corner of the intersection. The fall was due to adverse conditions above bolt anchorage.

An unintentional roof fall occurred in the 13I section in the intake entry. The fall occured at 13 crosscut and measured approx. 70ft long by 16ft wide by 12 ft high. No injuries or equipment damage due to this incident.
A roof fall was discovered in the 2nd Northeast Parallel in the #2 entry xc-18, spad #12+60. The fall was width-19.5ft,height-8ft,and length 24ft.
An employee was riding in the Somerset Portal elevator from the mine level to the surface. The elevator stopped approximately 100 ft. from the surface landing causing entrapment of the employee. There were no injuries or equipment damaged as a result of this occurrance.
As the weekly examiner was traveling an air course he encountered a roof fall in the return air course. The fall is located at spad # 2455 and extends outby 100', it is 18' wide and 5' high. The area was dangered and breakered off.
An unintentional roof fall was discovered during a pre-shift examination. The fall occurred at the #2 Entry of the Return, 1 break outby Spad #138. It measured approximately 6 -7 feet high, 20 feet wide, and 40 feet long.
The north drive motor for the 16' hoist failed.
A roof fall occurred in the #5 entry between crosscuts 86 and 87 in the mine's intake entry. The fall was above the roof bolt anchorage and measured approximately 100 feet in length.
A non-injury accident, the unintentional stopping of the Camp Run Portal elevator with 2 employees on board for 20 minutes. The employees were removed at 7:20 AM. The source of the malfunction was found to be in a set of contacts in a bumper switch. The elevator was returned to service at approximately 9:05 AM. Schindler Elevator Service was called to evaulate the occurrence.

An unplanned roof fall occurred in the #1 entry on J-3 panel 1 break outby survey spad #402340, appx. 8000' from the active face. No equipment or injuries were involved in the fall.
A fall occurred in the North East Mains at crosscut 225 between the #4 and #5 and includes the #4 entry on the belt line. No equipment, ventilation or personnel were affected by this fall.
An unintentional roof fall occurred in #4 entry of E-mians 001 section. The section was idle at the time of the fall. Fall was discovered on pre-shift exam. The fall occurred in an intersection & spread into the x-cut 4-3 & x-cut 4-5. The area was bolted with 6' fully grouted resin bolts, T-3 and 10' cable bolts in the intersection. The fall was 70' L x 18' W x 91/2' thick.
An unintentional roof fall occurred on the 4C neutral entries at #8 entry 9+80 1st. south sub-main off east sub-main. It measured 25'x 25'x6'. It did not affect ventilation, passage of men or cause personal injury. The area will not be cleaned up. It had been supported with 6 foot fully grouted roof bolts.
An unplanned roof fall occurred in the #4 entry of the K-2 panel, 1 bresk inby 20527, approximately 10,000 feet from the active face. No injuries or equipment were involved.
At 10:45 PM, a large flame shooting out of the vent on top of a salt water tank on a gas well pad was discovered. The gas well company (XTO) was contacted and the fire department was dispatched. The fire department responded to the scene and extinguished the fire 12:15 AM. It was determined that a lightning strike caused the venting gas to ignite.
A roof fall measuring 30 L x 18 W x 8 H occurred in the #6 entry at spad 1785 in the No. 2 SW Mains off of No.1 SW Mains approximately 8,050 form active mining areas 005 and 003 MMUs. Area was bolted with 5 TT bolts.
The elevator, which is the main entrance to the mine was not working due to a faulty connection of electrical wires.
An unintentional fall of roof occurred in the #8 heading, at or near spad #9739 on the 2nd south mains in the intersection. The fall measured 30' long x 50' wide x 6' to 8' thick.
At approx 6:00am on 10/8/10, at the time of the preshift inspection, smoke and smoldering material was discovered at the previous shifts work area. The previous shift, which ended at 1:30am on 10/8/10, had used a torch to cut material. A hot spot check was performed at the end of the shift. Upon discovery, the smoldering material was extinguished.
An unplanned roof fall occurred in the #2 entry on #9 Belt Panel at spad 4402. Fall measured 20' long x 20' wide x 6' thick. No equipment or injuries. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Area was developed 3/2010. Federal & State agencies notified immediately. There is no active section at this operation at this time.
Non-injury Roof Fall located at 60' inby spad #274, #7 entry (5' H x 15' W x 20' L).
A rockfall occurred in the #3 entry at brk.3, this is the #10 beltline, the size of the fall was approx. 25'L x 30'W x 8'-10'Thick. No ventilation or machinery was affected. Area is being cleaned and additional support is being added.
A rockfall occurred in the #3 entry on 16 beltline at break 10, the size of the fall was approx. 35'L x 40'W x 6'Thick. No ventilation or machinery affected. Area is being cleaned and additional roof support is being added.
While performing normal activites at the escape shaft, the hoist electrical controls developed a minor electrical problem. Two miners hand scaling inside the escape shaft at the time of the malfunction were brought to the surface. A mine evacuation was initiated at 12:45PM and miners were brought out to the surface. Maintenance repaired controls, hoist back up by 1:00PM.
A roof fall was discovered in the number 2 entry adjacent to spad 25065. Estimated size of roof fall is 10 foot high, 20 foot wide, and 20 foot long. This area was last mined in January 2009.
An unintentional roof fall occurred due to the strata broke at and above the anchor point. The fall occurred at Unit #1 alternate escapeway travelway in entry #4 @ survey station 3+75 and 4+40. Dimensions 18 ft. wide, 30 ft. long, 4 to 5 ft. high. Fall will be cleaned up and resupported.
A roof fall was found in 9 mains at brk. 31 in #5 entry, this is located in old works 5 brks. inby section. No ventilation or machinery effected, the area has been crib off on both sides of entry. The fall was approx. 20'W x 40'L x 5'thick.
A roof fall occurred at spad #2178 and measured 60'x18'x4' to 5' thick. The area was roof bolted with 4' long fully grouted roof bolts approximatelly 8 years ago. The fall will not be cleaned up and no injuries occurred.

Roof Fall did not affect ventilation and or passage of persons. The fall will not be rehab. Area was developed 9/10/10. Roof support consisted of resin roof bolts. Federal and State agencies notified immediately.
A fall of roof was discovered in the 3rd North West Mains at cross cut # 79 in the # 1 entry. The fall was 1 cross cut outby the section dumping point. The fall was approx 18ft wide x 18ft long x 6 plus ft high.
At 7:55 am the service elevator went down as it was approching the landing at the top of the elevator shat. As the elevator went into slow down speed it blew a fuse. The fuse was replaced and the elevator was put back in service at 8:30am.
Elevator's control cable was damaged entrapping 2 miners.
At approximately 8:45 the man elevator became in operable after investigation it was determined that a door safety switch had failed. The switch was replaced on the elevator.
The Highwall fell on the beam at Highwall Miner #2. Excavators had to be used to remove rock to finish retracking.
The spring on the dogs broke on the 14' mancage. It is being changed out. Cote Blanche Mine still maintains two escape routes out of the mine by way of the 8' and 16' shafts.
Roof fall was found on 1st left off 1 west sub mains, 18break-20break #4 entry, spad # 469. Fall was 110' long 19' wide 7' thick.
An unplanned roof fall occurred on the 2nd S. W. Sub-Main at cross cut #96, entry #5 on the conveyor belt. The area was bolted with 8' & 10' point anchor bolts. The roof fall will be cleaned up.
The mine encountered an unintentional roof fall on the 5A, XC32, from #0 entry to #2 entry. Dimensions: 160'L X 18'W X 8'T. Area developed in 10/2007 and supported with 6' fully grouted bolts. The fall will not be cleaned up. Approaches shall be timbered and dangered out.
While hoisting salt, the South skip dumped the load of salt and one of the pedestals for the dump arms broke. The dump gate opened while it traveled down the shaft and got wedged in the shaft. The damage caused the skip to be changed out.
Fuses blew on the transformer that powers the slope hoist. This caused the slope car to be inoperable from 9:30 PM to 2:00 AM.
An unplanned roof fall occurred near spad #1048 that measured 25' L. X 19' W. X 16' H. No injuries were reported. The area was developed on 10/14/08.
A roof fall has occurred in the #6 entry on the northwest panel #5, 150 feet inby spad # 1788. It fell betrween the blocks between the last and nect to last break forming the bleeder across the panel. The fall measured 18 ft wide, 50 feet long adn 3 to 5 feet high. Persons, ventilation, equipment nor travel were impedded. This area wil not be rehabilitated.
#2 hoist tripped out due to a PLC failure. Investigation was conducted by Maintenance Coordinator, Mine Maintenance Superintendent, and two electricians. A bad card was found in the PLC and replaced. There were two escapeways from the mine throughout the incident.
Mine Fire: A fire occurred in the No. 2 crosscut and the outby ribline in the East Main Belt Entry. Some material, old pieces of belt, wood belt cores and old rollers started burning. It appears that a spark or smoldering material was blown onto the pile of material. It was put out with fire extinguishers and water.
Unintentional roof fall in the 8N Bleeders, #1 entry, #39 crosscut. The fall measured approximately 18'w x 10'h x20'L.
Mine slope car/hoisting system inoperative due to moisture in e-stop connection in middle slave car. Moisture was producing nuisance tripping of e-stop. Found and repaired.
The 8 Haulage elevator door switch malfunction causing the elevator to be down for over 30 minutes.
Employee received fatal injuries from a shuttle car that struck him while hanging a check curtain on the #6 unit, #7 entry, #11 seam. His body was discovered by the car operator upon returning from the feeder. (AN MSHA, STATE AND COMPANY INVESTIGATION IS ONGOING.)
An over-the-road truck driver (not a miner, nor an employee) was pinned against a loading dock by a shuttle trailor that was backing up to the dock.
Adjusting and cleaning of a tail pulley on a 110' Masaba Conveyor LOTO was not performed by the EE before entering the tail pulley section of the conveyor. Belt was energized by the operator; EE was in tangled in the pulley from his feet to his mid section of his body. EE was fatally injured.
Employee had finished bolting a section of mine roof at approximately 10:30 am and was preparing to move the roof bolter out of the #1 entry when he was handed a wrench he had requested earlier. He walked up the side of the bolter to place the wrench in his tray when a piece of rock fell without warning, striking him.
The investigation into the accident is on-going, but as of now, it appears as the three employees were installing ground vault indicator lights when an ach flash occurred, injuring them. The employees were transported to the hospital.
Haul truck was dumping a load of dirt when the dozer operator approched from blind side to get driver's attention when he was struck by the truck and was fatally injured.
Drill steel was hung up in hole, pulling on machine came out quick and smashed ""R"" ring finger between rib and jack leg which cut tip of ""R"" ring finger.
Employee was on knees vacuuming floor near north packing stand door. Another employee pushed door open on his way out of packing stand, at that moment Employee vacuuming reached up and put his right hand on door jam, as door closed it caught the end of his right ring finger. Employee jerked hand back and a portion of finger plus nail was torn off.
Employee alleges he aggravated a pre-existing hernia while using a 16 lb. sledge hammer to loosen bolts from the #11 Mill Bull Gear. Employee alleges he felt pain and a tearing sensation to h is right side.
Employee was separating shim packs for upper bearing on main rotator of swing case #4 of a Marion 8750 Dragline. After heating the laminated shims, he was using a putty knife to work the shims apart. The putty knife caught and pushed the shim pack, slicing though his gloved hand causing a laceration to his left thumb and a cut tendon. *A 7% whole person impairment was issued.
EE went to check the pipe from a baghouse dust collector pipe to check if material was falling through the pipe. EE placed his hand inside the pipe and slipped while reaching for a flashlight causing his hand to go up into the pipe contacting the rotary airlock device.
Employee was installing resin bolts in browe of water ring. He was on level muckpile drilling. Strain/hernia.
Employee was working on an emulsion truck. The air acuator was stuck and he hit it with a wrench, causing it to move and pinch his right glove and index finger. He was seen at the ER and referred to an orthopedist. A portion of the bone was removed at the ER, and then more in surgery. He was released to full duty at his next shift on 10/14/2010.
OPERATING WITH THE GUARD DOOR OPEN, GLOVE GOT CAUGHT AND PULLED RIGHT ARM IN. CAUSED AMPUTATION.
Freight truck driver was binding a regular load of freight to the truck for shipment from the port to the mine site. While pushing down on the securing straps that bind the equipment to the shipping flat he felt a strain in his upper torso. He later found a bulge that was diagnosed as a Hernia. The company was not notified of the diagnosis until Tuesday 12th October.
Employee was walking down stairs when he felt a pop or pull in his groin area.
Operator was in a curve when his foot slipped off the brake and hit the gas causing the truck to hit the rib. When he hit the rib, his elbow hit the cab inside of the truck.
EE was standing under the bolter canopy, filing on the end of a drill steel when a rock fell, hitting the end of the drill steel causing the opposite end to strike EE above the left eye.
EE was lifting a pump out of a sump when the pump slipped from his hands. EE grabbed for the pump and wrenched the right arm and shoulder, starting at the elbow. EE continued to work until 1/10/11 when his restriction for transitional work could not be met.
Employee was crossing the coupling of 2 coupled rail cars, his hardhat came off his head and he hit his head on a step on the railcar that was there to access the hand wheel brake.
Employee was walking by the miner and a rock that was on it, broke and fell down hitting him on both shins and his feet. No broken bones, but doctor put him off due to swelling and pain in his right ankle.
He struck his hard hat on the mine roof and felt pain in his head and neck. *The operator disputes the allegations set forth in Citation #8051123 and submits this report solely to terminate the citation.
Employee was preparing to change vorsive basket, while removing lid on basket employee lost his footing which caused him to drop the lid mashing his fingers on right hand.
The employee, Production Supervisor, was examining the faces on the # 4 section and reported he stepped into a hole twisting his right knee. The company has not received any medical documentation on the claim but he is missing work.
EE was using a slate bar to free a pin on the scoop batteries when the stuck pin came free the weight of the batteries kicked the slate bar sideways and hit the EE in the face.
Employee reached for a 1/2 inch drive 1 1/8 inch socket
EE was assisting coworker in attempting to uncouple the car cat head, when his left foot became caught between the car pusher and miner guide rail, causing a crushing injury to his left foot. EE was flown to Hospital.
He was operating a jeep along the B mains haulage. The #6 switch was thrown for the curve. He applied the brakes and slid into the #41 diesel crane that was parked in the switch. He struck his right leg against the frame of the jeep. He sustained contusions to the right leg in the shin area.
The employee was attempting to climb back into the Volvo L200 loader he operates when he slipped on the lower step of the machine. He fell to the ground hurting his right leg. (knee)
The employee was in the process of extracting coal with a continuous miner when a kettle bottom fell from the roof causing him to immediately jump up and fracturing his ankle.
Employee entered a barricaded area where other employees were conducting maintenance and fell through an open hole in the floor, falling approximately 8 feet. Employee experienced some back pain, but did not receive treatment. Employee's doctor determined on 2/18/2011 that surgery may be necessary.
Employee cleaning out plugged pipe with air lance decided to check plug by inserting his left arm inside the pipe. Material fell down from inside the pipe hitting the employee's left arm. Left arm bruised and sore.
Employee stepped over a rock and twisted his right knee. EMPLOYEE DID NOT START LOSING TIME UNTIL 11/09/2010 DUE TO INCREASED PAIN.
The injured EE was assisting a coworker with a derailed flat car and the EE was attempting to decouple a flat car and he placed his left foot on the draw head, as the drawhead kicked to one side, his foot was caught between the drawhead and the flatcar.
EE was walking underground when he felt a pop in the back of his left foot. There was no trauma or event that caused this to occur. He had been experiencing additional foot problems and was being treated by a doctor. He has had surgery to repair the achilles tear. The doctor reports are attached as well as a history on the employee.
EE was getting onto the man trip when he twisted around to get on he felt a pop in his left knee.
EE was changing battery on Mac 8 mantrip. Battery arced and caused battery to burst.
Employee had been roof bolting, stopped and walked beside the miner; while talking to the miner operator, a piece of rock struck him on the head.
EE was descending from the CB Paradise using the ships gangway as he was stepping from the gangway to the cross over bridge, the gangway shifted. EE felt a pop in his left knee while the gangway was moving. EE was taken off work on 11/02/2010 pending surgery.
While parking Grader near end of shift, employee noticed the small door on the right rear at radiator was opened. Holding to hand rail, employee walked to door and while bending over to shut door, his feet slipped and all of his weight was on his left arm and shoulder. He went home and next morning went to Dr. He was taken off work.
EE was in an enclosed cab of an excavator, loading rocks into the bed of an articulated truck when the rocks accidentally fell from the bucket causing them to fracture. Random flying rocks crashed through the wind shield hitting ee in the knee. He was treated and released for severe bruising of his knee at trinity medical center .
Employee was moving the section waterline manifold and when he placed the manifold down and against the mine rib, the manifold slipped and mashed the end of his right middle finger between the manifold and the mine rib.
EE was watching the scoop to push the rail on rail trucks, the rail got caught on Fender of scoop. EE and another ee decided to push rail over with slate bar. EE put pressure on back of the rail while ee took bar and attempted to slide rail over. The rail slid off the side of the scoop and caught his leg underneath it.
Employee was assisting in setting a newly constructed loadout building onto the back of the boom truck. Rigging came loose on hoist causing the loadout building to shift causing the employee to fall to the ground. The employee fell approx. 54 inches resulting in a broken right wrist and a fractured little toe on the left foot.
Miner operator was cutting in the No 2 entry when his cable was sliding under the miner. The operator hooked the cable with his right heel of foot to pull the cable out of the way. He felt a sharp pain going down the side of his right leg.
While plastering a stopping injured got B Bond in his left eye. He did not seek medical attention but according to Pay Roll records he did not work the day following the incident
Pulling Rock out of #7 Head Drive. When lifting rock, felt sharp burning in back.
EE was climbing into bed of triaxle, reaching on top of bed with hand. Lost grip and fell striking another truck, ground.
Employee was cleaning off the bolter. His hand and forearm grazed the strap cutting him on the top of his forearm. He was bandaged then transported outside. From there he went to the hospital where he received 13 stitches and was told he could possibly have nerve damage.
Employee was lifting dryer basket to remove socket from underneath when he felt pain in his back.
The employee was setting and seated in the rear of the diesel powered mantrip going underground to his section. The mantrip bounced causing the employee's head to contact the stationary handrail. The incident resulted in lost time injures.
Going down hill on New Road. Ran over something, tore out plug in bottom of fuel tank & lost all the fuel out. Motor died out, lost brakes. Truck was checked at the scene. Was driver's fault.
While installing a 48"" diameter fiberglass 'Y' for a ventilation pipe, an employee was pressed against the railing of a personnel lift and suffered a hairline fracture to the #9 rib on the right side.
During a closing of a septic tank system at a residence in the town site a contractor backhoe moved a dead wood log disturbing a hive of bees that were in the log but that were not evident during the area inspection that morning. The bees came out and attack the employee who was a bystander 60-75 feet from the log. The employee recieved multiple stings on arms and neck.
EE was operating loader when it slipped off a large rock, causing his right shoulder to strike the dash and windshield. He was taken off work effective 10/28/10.
Employee was getting out of a scoop and lost his balance, he hit his head on the rib. This caused him to sprain his neck. He began to miss work on 10-29-2010.
EE was unloading supplies and carrying a bundle of roof bolts. EE stepped in a hole injuring his right knee.
While using chipping hammer a piece of ore flipped up into his eye.
EE was holding a 3' bar while I was trying to pound a small pin into the end of the roller for a conveyor, when I hit the pin a small piece of metal split off the pin into the corner of the EE's eye. Had to take him to the doctor to get it removed.
Concrete form was lowered onto the injured party's lower back, resulting in a compression fracture of a vertebrae. The injury was not immediately known. The injured later saw a doctor.
Injured worker was water blasting and felt a sharp pain in his lower back
EE was walking to the continuous mining machine to help clean the scrubber screen when the corner of a pillar block rolled off and struck him on the right leg. The rib roll caused a fracture to his right foot.
Two employees where engaged in horse play, one employee went to lay the other employee down and while doing so it broke his left fibula and ankle.
Stated: The employee was in the process of changing out the springs on the feed end of the A2 desliming vibrator. The vibrator was being held up with a 3 ton come along hooked to a loop of 3/4"" cable thru another metal plate. As he was putting the new spring in place, the cable pulled thru the metal plate. The spring mashed the small finger on his right hand.
Employee tripped over bolts that were mounted in the ground that held the tugger for the skip replacement job. Employee wrenched his neck and shoulder.
Employee slipped and twisted back while going over berm to start and fuel pump. Heavy rains overnight.
EE alleges he fell off a front end loader on 10/27/10 sustaining multiple injuries including a closed head injury during his course of employment. EE did not report this on date of alleged injury. There were no witnesses. Supervisor denies there was an injury. Employee filed application for adjustment of claim on 1/5/11. Employee was terminated on 11/1/10 for attendance issue.
Employee was taking pop cans to container, stepped on cover of man hole. The cover came off and employee fell into man hole.
Employee hammering on a shuttle car reel chain and a piece of metal flew off the hammer striking him in the right upper leg.
Subject alleges he was hanging coal drill cable to roof bolt plate. The drill operator engaged cable reel and it struck subject in rib cage. Bruised and possible fractured ribs.
EE injured his left shoulder while lifting bottom roller back into hanger. He refused medical treatment at end of shift. He was contacted at 11 am & still did not need treatment. At 2 pm he called mine site & stated he was not going to seek medical attention. He called back around 8 pm & stated he had gone to doctor & would not return to work on his next shift.
The employee was ascending a ladder to tie off the ladder. The ladder was on to flat of an angle and the employee did not seek assistance to hold the ladder while tying it off. The ladder base slid out - the employee reached out landing on his left arm.
Stated: The employee was moving the bolter cable while the bolter operator was backing the machine up. The operator turned on the cable reel, jerking the cable out of his hand and causing pain in his shoulder. He bolted 10 places and finished his shift. Medical evaluations show tendinopathy and joint separation.
EE lost his balance while using a water hose and reached with his left hand for support, grabbing a belt roller smashing his left middle and index finger tips.
EE was in the process of removing a wooden ladder from the top-mounted ladder truck of a pick up truck. EE stepped off of the running board to the ground (approx. 12"") and twisted her right ankle.
They were helping to change teeth on the dragline. EE was watching while the foreman was trying to knock the teeth out and a piece of the tooth chipped off and hit the EE going into his right leg above his knee.
Riding mac-8 inside, he was sitting on knees with back toward the top when a roof bolt hooked his bibs pulling him off the mac-8.
He was walking along 10 belt when he tripped and fell striking the right side of his chest on a belt roller. *The operator disputes the allegations set forth in Citation #8045764 and submits this report solely to terminate the citation.
Employee was leaving the cab at the end of his shift. As he walked down the steps he had his left hand on the hand railing and on the right side was the motor compartment of the machine. When he stepped on the platform his feet went out from under him. Resulting in him falling backwards onto the steps.
Employee was pulling up on a bar when he felt a sharp pain in his lower neck and upper back area.
EE stated that ""while pulling on rubber mat that was caught under the 1st Rt. Power Center"" he felt pain in his right shoulder blade area. *EE continued to work this shift and did not miss work until 10/28/10.
Employee was pushing a shield onto the hoist and foreign material got into his right eye.
EE fell and landed on his hand. This report is being generated under protest to terminate citation #9006085.
Ee was standing beside the rib, when a piece of rock fell from the roof and hit him in the head and shoulder. Injuries are numerous but not life threating or debilitating. Doctors are saying he will be off for two or three months, maybe more.
Air jumping the fuel trailer with the Eimco. The hose was not hooked up. When the air was turned on, fuel sprayed into worker's eyes.
Employee was trying to remove a rock that was jammed between the conveyor frame, the belt and the return roller. Employee was using a pry bar and while he was trying to remove the rock the bar slipped. The employee lost his balance and while he was trying to catch himself he strained his left knee.
EE dropped a water hose to a concert floor of the bunker engaging the valve causing it to flip up and strike his left eye area.
Man wth hist of chronic rt ankle prob rolled Rt ankle & treated 4 grade 1 sprain able to place all weight on ankle & walk unaided. Missed lge amount of work 09 due to severe sprain at home. Stated was having pain in ankle weeks prior to this injury. Already had an appt with his Dr for evaluation of chronic pain. Also wears a brace on rt ankle & has done so since off the job injury
Employee was bolting #4 entry. When he went to straighten roof bolt back in pot on bolter. He felt a pain in lower abdomen, and groin area.
The ring finger on his left hand was pinched between two pieces of metal. When screen was rotated around, finger was caught. There was no conditions that contributed to accident.
Injured member was walking and conducting his fireboss duties when he slipped and fell. He attempted to break his fall by placing his hands in front of him and injured his right wrist. He has been diagnosed with a wrist fracture and has been excused from work.
Employee was pushing a hose clamp up on a hose with a screw driver and screw driver slipped hitting him in the right eye.
Employee was taking mantrip off charge when it started to roll and the power cord was pulled out of power center. Employee tried to stop it by grabbing the cord with his hand, jerking his arm and shoulder.
Tried to raise 6"" plastic pipe from chain to drop it down. He felt pain in his lower back at that time
Mill Mechanic was lying down on a concrete floor and reaching through an opening while pulling on an 18"" pipe wrench with his right arm. He was trying to break loose a rusted pipe fitting on the bottom of the air receiver tank. Between the awkward position and the force required to break loose the fitting he suffered an injury to his shoulder.
Repairing ram car wheel. Another employee operating separate ram car struck a 4 wheeler and then it rolled into the ram car EE was repairing striking him in leg.
EE got his finger cut with the edge of a 12"" pipe while operators were performing maint. on the dredge. The injured finger is the right little finger or ""pinky"". Operator was wearing his P.P. E. at the time including gloves. EE was taken to the hospital immediately and doctors will determine the damage.
While crossing the inby airlock door, someone attempted to open the outby airlock door causing an increase in air pressure on the inby airlock door. In doing so, the 2nd door of the inby set of doors tried to close and struck the employee on the right arm.
Employee has been diagnosed with inflamation of the sciatic nerve
Employee was riding to the section in a Lee-Tram when it hit a hole in the roadway hurting employees back. Employee did not start missing time due to this accident until 12/03/2010.
While operating an open four wheeler, the subject hit his mouth on an open door in the secondary escapeway. He cut his lip and required 10 stitches to close the wound. Height is approx 45 inches.
Injured member was lifting a pump at the bottom of the slope, when he felt a sharp pain in his abdominal cavity and he noticed a small discharge of blood from his rectum. He has not returned to work.
Employee was lifting his arm straight up to grab onto the straps from overhead crane to attach to a bag to be lifted. When he brought his arm down, felt a rubbing sensation in left shoulder area. When employee attempted to raise his arm again he felt a sharp pain in his left shoulder area.
The EE got down on his knees while operating the CM. When the EE started to stand up, he stated that he had pain on the backside of his knee. The EE stated that he did not know what happened that his knee just started hurting. The EE cont. working until 11/09/10. Note: The employee did not start workers compensation until 11/09/2010.
Helper side roof bolter was installing a new row of bolts. As he was swinging drill station toward the rib to install the #1 bolt, a piece of rock hit the canopy, then hit his back and right leg, knocking him to the floor.
Employee was at slope bottom lifting a safety rope onto car. Employee felt a pain on left side of lower back into the buttocks.
Employee alleges that he struck his knee while moving around in the cab of a piece of equipment. The employee failed to report the injury until 11/14/2010 and then sought treatment with his personal doctor on 11/29/2010 where he was diagnosed with a torn meniscus, received a Cortisone shot, and was prescribed two (2) days off from work.
Operating 700 polaris (side by side) in an unsafe manner. Going too fast. This accident did not occur on property under mine permit.
The employee stated that he was traveling around a corner in # 7 entry in a battery coal hauler, when his front wheel climbed on top of an uneven ledge. This caused the coal hauler to slam down jarring him in his seat. He stated that he was having lower back pain and right leg pain.
EE was hanging the continuous miner cable when he felt pain in his left shoulder.
Employee was hanging miner cable in No. 3 Entry, felt pain in lower back.
ee was putting up a hanging bracket for a screw conveyor, while hanging bracket, finger was pinched, tearing the end of his finger. ee recieved 2 stitches in end of finger.
Operating 700 polaris (side by side) in an unsafe manner. Going too fast. This accident did not occur on property under current mine permit.
Standing on ladder, EE slipped and fell to the floor. Landed on buttock. Acc. occured on 10/21/10, did not seek medical attention until 11/12/10. EE has been off work since 11/12/10.
Alledgedly hurt his back while loading 10/4 power cable into a p/u truck on 10/21,regularly off 10/22-24,worked 10/25,reported accident 10/26,worked 10/27,10/28 went to workers comp Dr.and was wrote out of work.First day missed 11/1/2010.Witness did not see him get hurt or show signs or was not told that he hurt himself. 11/1 mine idled workers transfered to another location.
Employee reported while loading dragline bucket teeth into a truck he strained his shoulder. Reported to employer on 10/21/10. He was restricted from full work duties as a result of this injury on 10/22/10.
Driller was slabbed resulting in fractured scapular & 2 broken ribs on left side. it was determined that a slab or chunk of rock had fallen from the face and struck the miner while he was bent over painting up the face. The heading was in slaty, graphitic argillite which had been spiled on the previous access with drill steel to support the heading.
Light went out. Sent to get new light from foreman & fell in hole.
Reportable Only due to sutures. Employee was helping move an 8"" steel water pipe, when Little right finger got caught between the pipe and a light bracket. **Employee did not start missing time until 10/26/2010.**
EE walked behind the continuous miner to check his centerline. While walking back to his position to load another sc, he tripped over the miners waterline and fell on his left knee. He finished his shift, and worked the remainder of the week, but his knee continued to bother him. Went to his doctor 10-25-2010 and was diagnosed with a sprained meniscus, taken off work for a week.
The employee stated that he exited the mantrip, reached for his lunch bucket, and as he started walking out of the mantrip shed he tripped over a bucket of pinner bits and fell onto his dinner bucket. The employee injured his left wrist and ribs. An x-ray showed a fracture to the radius. The shed had a concrete floor and was wet. The bucket was sitting 20"" from the paarked mantrip.
Individual was cutting a beam out on the second floor of JWR 5 Prep Plant. When it came loose, the beam struck him in the face.
Employee was using a pry bar when it slipped and hit his thumb.
Wet weather, mud on the boots of skidsteer operator. When he exited the skid steer, (New Holland 190) he slipped off the forks and was falling. He caught himself before he hit the ground twisting his right hip and back.
Injured was walking into bathhouse and slipped on steps, hurting knees and back.
While the employee dismounted his Cat 740 haul truck, he twisted his right knee when his foot slipped off the last step. The employee did not seek medical attention at this time. On 12/07/10, employee requested medical attention. Light duty was issued by a doctor which made it MSHA reportable.
EE was operating the head drum of the long wall shearer a rock was thrown from the shearer drum striking the ee on the chin and mouth. EE received 2 broken teeth and 2 cracked teeth.
Unhooking high voltage cable, cable was double hooked on cable hangers, after he released the first hook the second hook straightened out and struck him in the mouth cutting lower lip, knocking out one tooth and breaking another tooth off. He began loosing time the next shift.
An employee was changing the injector pump on #11 forklift in the shop. He was utilizing a long handled 1/2"" drive ratchet to turn over the motor on the forklift to align the timing marks. He was standing on the balls of his feet, leaning into the forklift when he felt pain in his left calf muscle. He sustained a tear to the upper calf muscle and was placed on restrictive duty.
EE was operating a fork-lift and it slid over, causing it to catch the bottom edge of a spreader stand and hit him in the cheek. EE has sustained a fracture in his face and must be off work for a few days.
Employee injured his right shoulder while shoveling the tracks on a dozer. While removing the dirt from the tracks with a shovel, the shovel contacted a bolt and quickly stopped the shovel and injured the employee's right shoulder. The employee had surgery on his right shoulder on January 03, 2011 and he has been off work since that date.
Pushing a #6 gear box on, when it stopped suddenly, hurting right shoulder.
Employee was exiting a personnel carrier when the tip of his boot caught the edge of the personnel carrier causing him to fall. This incident did not become reportable until 11/29/10.
EE caught his left ankle between the rib and a rail motor and fractured ankle. Started missing work 10/2/10.
EE was working on a conveyor belt in a kneeling position, when he stood up to re-position himself, he felt a pop and muscle strain in his left knee.
Pulling rock out of bridge conveyor and cut right 5th digit finger. Started missing work 10/20/2010.
EE was loading coal from #5 heading when a small piece of draw rock fell on the side of his face. When he turned his head to get the piece of draw rock off, he twisted and hurt his neck. EE continued working and worked until 11/5/10.
Was decending a haul road with a loaded truck, he went to change gears and could not get the transmission in another gear. He turned the truck over onto the passenger side. On an approximately 8 percent grade. This accident is a contractor for Huffman Fork (LB Trucking #K193) but we are required to report the accident according to our MSHA district.
While loosening a walk side cover bolts, employee felt a pain in his right shoulder. Employee has been treating for this injury since 2010. He will now lose time starting today, 1/29/15, since he is having surgery.
While pulling on trolley chain to move overhead trolley the trolley stopped suddenly causing the injured to feel something pop in his left hand. The injured suffered a fractured left hand. The injured continued to work and went off work on 10/30/2010 when an MRI showed he had a fracture in his left hand.
EE was walking when he slipped and fell. He has a chest wall bruise. This causes pain, but no physical problems. A few days of rest is all the doctor prescribed.
Contractor stepped in a hole and twisted his ankle when walking across L.O.C.
EE stated that while he was standing beside of the roof bolting machine the machine rolled back and ran over his left foot.
Employee became disoriented and was walking along mine road. He was taken to local hospital, then transferred to Austin for further testing. The hospital has still not drawn any conclusive reason for his being disoriented. Perhaps medication issue. He is being seen by a neurologist and his pcp. He has not returned to work as of this time.
Two employees were carrying a shield power supply, when one of them slipped on the mine floor and felt pain in his lower back. The mine floor was muddy in this area and the power supply weighed approximately 80lbs.
Employee was removing pin from monorail. Rib rolled into entry. Employee was startled, moved quickly slipped and fell onto rolled out rib injuring his left thigh area. Diagnosed with left thigh contusion. Started missing work on 10/26/2010.
Employee was changing a tire on a light duty pickup truck when he twisted his ankle.

While lifting corner of marble block with excavator so ee could place block of wood under the block of marble 4 piece of marble fell off. Striking ee in the crest and lower neck ee had wind knock out of him, and sustained bruise to his chest and neck area.
EE was leaving the work place after quitting time in his personal auto. Another private auto was moving on the same road and they hit head on, causing the auto accident.
EE dayshift foreman,was welding using a welding hood that belonged to someone else. The hood did not provide adequate protection for ee eyes. EE eyes were irritated due to the welding. EE was written off work for 10/18/2010 and returned to work on 10/19/2010.
Section mechanic positioned himself on the right side of the roof bolter to begin repairs on a bantam rockduster. The right side bolter operator finished installing two rib mesh bolts and swung drill mast in and back in order to move roof bolter. Left side bolter operator found mechanic caught between bolter frame and right side drill mast that was swung back in place.
Employee was riding out of the mine on a lube truck when he felt some discomfort in his abdomen. He did have a pre-existing abdominal hernia, however it was determined by three different physicians he had an abdominal muscle strain which was the issue, not the hernia. The determination of the muscle strain was determined on 11/3/2010.
Employee was operating fuel truck when he hit a bump causing him to be jarred.
The employee was utilizing a rock hammer to tap on a slip-fit packing gland located on a shuttle car tram motor, when a piece of metal broke off the hammer and embedded in the employees chin. The employee sought medical attention and the piece of metal was removed and the wound closed with sutures. The employee has been placed on restricted duty until the sutures are removed.
Employee was assisting in installing bottom belt roller. Another employee picked end of roller up & pulled EE slightly forward straining his back.
Employee stated that he was in the outside parts trailer putting away a splice kit when he turned to leave the trailer, he stepped on a screw jack assembly causing it to come back striking his left knee. Employee felt a ""pop"" when it happened. Employee continued working regular job until 2/15/11.
Employee was cutting a piece of cable to install a belt switch during a belt extension on #4 unit. A piece of gob fell striking his back then his leg. The injury resulted in a fractured leg.
Employee was removing grizzly bars. When he removed a pin, the bars fell striking him in the side of the face and neck.
Operator was digging drainage ditch and was attempting to remove a rock, rock slipped out of excavator bucket and caused machine to whiplash operator from side to side. Operator complained of pain in lower neck and back.
Driver was bent over hooking up the product tube on the back of the trailer when he felt a pain in his lower back

Injured employee was working inside #377 752 Page Dragline. The machine was down for maintenance. EE slipped on a spot of grease and fell dislocating his right shoulder. EE was taken to Ohio County Emergency Room where his shoulder was put back into place.
Employee picked up the bottom stand while working on the belt structure.
EE was using a wrench and cheater pipe on a transmission bolt. The wrench slipped and struck him in the face/mouth/teeth.

EE was transported to the Logan Regional Medical Center by ambulance where he was released and referred to St. Francis hospital for surgery.
Employee had moved up scoop charger and was hanging extra charger cable when he felt something pull in his groin area.
Employee was burning ludges off conveyor on a shuttle car and when ludges became dis-lodge a flame came out and burned the left side of employee face.
EE was cutting a bolt from a steel beam with a cutting torch. A hot coal fell on the bottom of his pants burning a hole on his pants then falling into th etop of his slip on steel toe boots. He received medical treatment on the burn on his foot. He was told to take two days off from work.
While operating LHD #2-6 in 4600 Up Ramp, rock came off rib, striking employee on right forearm.
Piece of draw rock fell scraping EE on the face causing a laceration to his lip and also scraping both of his legs causing a laceration to his left leg above the knee. This injury required stitches to his lip and leg.
EE was preparing floor grating for replacement by trimming the flat metal that was installed at the butt ends to aviod a trip hazard, when the piece he was standing on gave way and allowed him to fall.
The man was using a pry bar to align the hole to insert a boom jack pin. While pulling on the pry bar it broke and the bar struck him on the cheek and eye.
Employee was riding in passenger seat of mantrip. Mantrip was being backed up and struck an 8"" water line hung from the roof. Employee felt his back pop. Employee is doing physical therapy. Crosscut 26 of 3 north roadway was the location of the accident.
EE was dismounting hauler, he missed the hand rail with his left hand as he came down the ladder he fell backwards to the ground. His right foot slammed into the ground.
EE was walking around side hill on nature soil/rocks and a rock rolled under his feet, as he fell he tried to catch himself with his right arm the impact jammed his shoulder causing it to dislocate.
Employee was running the dredge and while sitting in the cab he felt something on his hand. When he looked to see what it was, he saw it was a black widow spider but before he could knock it away, he was bitten by the spider. He was kept in the hospital over night for observation and was released to go home the next morning and was instructed to rest for 2 days.
Injured was transporting chemical tank when hyd. hose came off causing pond treatment to splash out hitting face and eye area.
Employee stepped between two longwall supports to allow another miner to walk passed. When the employee stepped back out from between the supports he lost his balance and fell down. His right foot was still between the supports pontoons when falling causing injury to his right ankle.
EE was bolting top and while pulling drill steel down, he hit the fast feed and dropped the head of the roof bolter on his knee.
EE was unloading 6"" water line off flat car with scoop. One joint of water line became caught on scoop ram. He attempted to free it with a slate bar. The pipe popped loose striking the slate bar which resulted in the bar striking his finger. The result was a dislocated finger with medical treatment only of a splint applied. No days lost.
EE was cleaning coal spillage under the re-claim belt and was walking along an uneven surface. EE twisted the left knee and complained of soreness to the knee. EE continued to work until 12-1-10 when he went off work.
Employee was taking a strap off a bundle of roof plates when he hit the top of his left hand against another bundle. This injury required stitches.
EMPLOYEE WAS CHANGING A TIRE ON A PIECE OF EQUIPMENT. HE WAS ROLLING THE RIM FLANGE AWAY FROM THE PIECE OF EQUIPMENT AND SOMEHOW FELL FACE DOWN ON THE RIM FLANGE.
Member was welding and looked at the arc and accidentally burned his eyes. He was unaware of this until he went home and tried to sleep. His eyes began to swell after a few hours.
While working underground, the employee felt discomfort to his left foot. His boot was removed and a red spot with red stripes extending from it was observed. Apparently the employee was bitten by a spider resulting in an occupational illness. Due to the bite, the employee started missing work on 10/13/10 making this occupational illness reportable.
Service Technician was standing along side his service truck. At a distance of aprox. 30 feet another crew was installing a track on a Hitachi EX600-6, using the prescribed method when a piece of metal flew across the 30 feet and struck the employee in the neck.

Employee connected two drill steels together and was pushing them up with the drill pot, when the steel popped out and struck him on the right wrist and hand.

EE was assisting in replacing the long cylinder on #7 press. He climbed down to the shelf of #7 press to cut the nut holding the long cylinder. When he cut the nut the press jumped and he stepped back and slipped off the shelf and slid into the hopper about 8' down. He fractured a bone in his right leg. His first day of lost time is 10/12/10.
The employee was unblocking a feeder duct with water and air. When the employee applied air pressure, suddenly hot water comes out of the duct affecting his right forearm resulting in first and second degree burns.
EE was bolting on the right side of the miner in 8 South. He was moving the drill head to unplug it and he had his right hand on the mast and it pinched his fingers.
Crew decided to place a rope spool on its side. As crane operator lifted, per signal person, top of load began to slighly lean towards the crane. Load was steadied by tagline. As he attempted to exit area, the bottom of load slid towards outrigger, hitting him in back of leg. The motion pushed his leg against outrigger, causing it to be pinched between the two.
Walking in Roadway, slipped in rut and broke right ankle.
Employee was collecting ore control sample and stepped on a rock and lost footing and twisted his back and he suffered a lumbar strain.

He immediately ceased work and was trasported to the surface to an awaiting ambulance. The ER physician diagnosed injured with a strain/sprain injury of his back and was excused from work for 1 week.
EE was trying to maneuver a 55 gallon drum of used oil into place with a co-worker into the back of a pickup when EE felt a strain in his shoulder. EE was taken to the clinic and released back to work. On 2/7/2011 EE was taken off work due to restrictions that were placed by doctor.

While shooting a snowman from the firing hood the 8 gauge gun malfunctioned causing a missfire to occur and the slug struck the metal plate surrounding the poke hole and fragments struck an employee.
The investigation into the accident is on-going, but as of now, it appears as the three employees were installing ground vault indicator lights when an ach flash occurred, injuring them. The employees were transported to the hospital.
Employee slipped and fell from blasting rig, injuring his ankle and tail bone.

Employee was moving a cover for 7-A longwall headgate drive when another employee slipped resulting in injured EE pinching left hand in between the cover and the headgate drive. Original trip to hospital was diagnosed with a contusion of the left hand. Employee then went back to the hospital on 10-12-10 & was diagnosed with a fractured scaphoid bone in his hand.
Employee was on the off side of the bolter getting ready to bolt when a piece of rock fell striking employee on the head, right shoulder & hand.
Bag house #1 tube had coal fines builing up and falling down chunks into the Jig, EE was reaching in and pulling out the chunks of coal fines and strained his neck.

We were cutting down a 30' section of conveyor and IE was making what he thought was the final cut, but he over looked one 2"" metal water pipe on the catwalk so when he made the cut, the 2"" metal water pipe came up and hit IE from behind and injured his back and face.
Employee was advancing panline when a piece of rock fell from two supports and struck top of right hand. Employee was examined/x-rays and was told it was only bruised and released to return to work. On 10/23/2010 the employee was re-examined/x-rays and was told he had a fractured bone in his right hand. Hand was splinted and he was released to return to work.

Employee is a dozer operator. 9 hours into the shift, the employee experienced back pain similar to previous incidents. The employee was unable to complete his shift. On October 9, 2010 he was examined at the Colstrip Medical Center and was unable to report for his next scheduled shift. The employee has a history of back pain incidents dating back to 2004.
Employee was operating loading machine on 9A section. A rock fell from the top, hit the loader canopy and struck employee's left middle finger, resulting in a fracture.
Lube Truck operator was pulling hose off reel on lube truck and stepped into a hole, twisting his left leg and hip. Starting missing work on 10/11/10.
One end of a chain was hooked to a 160 excavator and the miner hooked the other end to a hitch to move dolly out from under portable crusher. The chain came undone and the miner grabbed at the chain before it had quit moving. The chain pulled his right thumb through the eye of the dolly. The miner pulled his thumb out of the eye.
While driving a 777 Haulpak on the haul road, EE fell asleep and ran into a 345 excavator parked on the left side of the haul road.
EE was unloading 5 gallon oil cans off a flat car. He stumbled and fell to the ground, hurting himself in the groin area. He started to miss work on 10/8/10.

Stringing a conveyor belt onto a conveyor for vulcanization. Reached end of belt and while attempting to remove belting from the wood core the belt recoiled which caused a nail that was hammered through the belting to cut employee's forearm. Affected area was right forearm below the elbow 5"" cut.
The EE alleges that while he was fixing a stuck belt and pulley the belt rotated and pinched his finger.
EE was pushing a stone on a manual roller conveyor to feed a hydraulic stone splitter. While pushing the stone he felt pain in his groin area.
Working on approach to 3 South seals, using a sledge hammer to break up a rock. Hit the rock 2 times and felt pain in his right shoulder. It was not determined as lost time until 10/11/2010.

Employee strained his left shoulder while working on the #2 finish separator. Employee initially declined medical attention, but when the shoulder pain did not go away he went to see his doctor. This injury became reportable on 11/10/2010 when the doctor diagnosed the injury as a torn rotator cuff. The employee was placed on restriced duty. No time lost to date.
The employee slipped while walking to do fire extinguisher inspections. During which time he leaned to the left, straining the muscles in his lower back.
Employee was working around the Highwall Miner when flash from welding got into his eyes.
Employee was climbing down from operator compartment of a wheel loader when his foot on the ladder slipped. As a result he fell about 3 feet to the ground landing on his buttock and left hip. Weather conditions were sunny and dry. Employee is currently waiting for MRI test due to pain in back and legs.
When the employee stepped off a 10"" I-beam that was laying on the ground he stepped on a rock and twisted his left ankle.

While removing compressor rope, using a sheave pulley that was with original compressor setup, EE was stationed outside the pinch points and was the signal person for the operator on the Mini Excavator. This 7000-1 is submitted under protest only to abate citation No. 8503161. River View does not believe this is a reportable injury. It is believed that the injury reported on this date is the same condition as reported to MSHA from a June 24, 2010 injury. The employee reported that he strained his back on June 24th and on October 4th, 2010.
Employee was climbing into rock truck to begin shift. He twisted knee causing swelling and fluid in knee. Injury got worse by end of shift.
EE was being loaded when the bucket of the shovel opened dropping muck on the dove tail. EE was taken to the ER and taken off work for one day.

Employee was cutting a hole in a piece of belt when the knife slipped and hit him on the right leg two inches above the knee. He was going to use the piece of belt to drag rock dust to a trickle duster a few breaks away. Three sutures were used to close the laceration.
EE was walking to B Tube when he stepped onto uneven grating which caused him to stumble and fall onto his right knee. His knee became bruised. He did not start missing work until 11/1/10 when he said it began to hurt. He is off seeking treatment. This was reportable as a file only until 11/1/10.
A production operator sustained burns to his right arm, face and abdomen due to steam being released when opening the top access door on Crystalizer CZ-451. When the employee opened the door steam at a temperature of 148 degrees F was released from the crystalizer which came in contact with the employee.
EE called me at home, informed me he was getting treatment for a prior injury he sustained at another mine to his neck. EE worked his Sunday shift 10/10, and missed Monday to present. EE RTW on the 14th to pick up his check and said that he had put his shoulder to an airlock door as he was leaving work on 10/4/10, landing on his right shoulder aggravating old injury.
EE was stepping across an empty pallet, when a board broke and his foot fell through the pallet, twisting his right knee.

While helping set bits on the continuous miner the ee started down the #3 entry and was struck by a piece of rock that fell from between the bolts and pushed the ee to the floor striking him on the shoulder and left leg. The injury was not known until 11/9 due to a previous injury, when an MD read a MRI revealing a fracture to the tibia.

He heard a flyboard crack and then rock knocked his hardhat off. The rock hit his left shoulder and then rolled down to the back of his leg. His leg and foot were bruised. He was instructed to keep it elevated for three days.
EE was de-lugging a track tie and missed causing his forward motion to overextend his swing causing low back pain.
EE said his seat in his D11 dozer bottomed out and hurt his back.
Employee, was a passenger in a Kubota, - while traveling down a ramp, the driver lost control of the Kubota, striking the left rib, and the Kubota turned over on its side. EE sustained a right patella fracture.
Employee was moving a portable ramp that had folding legs underneath. Ramp was positioned against the wall and as the employee started to lay the ramp on the floor a set of the folding legs fell out and caught his left hand index finger in a pinch point. Suffered a severe laceration and broken bone to this finger.
Employee had completed shift and was crossing parking lot to get to his vehicle; this was his last day on the job. He fell and injured his left shoulder. On the way home employee stopped at ER. Examination indicated a dislocated left shoulder.
Employee was a passenger in a Kubota - while traveling down a ramp, the driver lost control of the Kubota, striking the left rib, and the Kubota turned over on its side. Employee sustained a right wrist fracture & bruised right inner thigh.

Miner entered Feed Shelf. He began picking up, removing material by hand and throwing it into the kiln. While moving back and forth removing chunks of material as he went, he lost his balance and stepped backwards into a pile of dust. He sank approximately three feet into the dust causing 1st, 2nd and 3rd degree burns to his right and left ankles and calves.
Employee was in the process of replacing shield over the pump compartment on a shuttle car. He placed the shield on top of the frame of the shuttle car. The shield began to slide off, as he attempted to catch it he lost his balance and fell backwards on the ground holding the shield. He experienced soreness and tenderness in his groin, knees, and back.
The man was walking in the section when his left leg gave out from under him.
The miner was stepping down from the back of a MMU Bulk Truck #126. His foot slipped off the ICC bumper and fell to ground. The miner tried to cushion his fall by reaching with his right hand and broke his wrist.

Employee was standing on a rock truck ladder and reached out to clean off the headlights when his feet slipped out from the ladder causing him to fall to the ground, dislocating his right shoulder.
The employee was roof bolting. He raised the pot up to the starter steel, which was not properly aligned, it snapped back jarring his hand.
Employee closed elevator door on hand causing his right index finger to become broken.
Employee was helping install an air lock door approximately one break inside the mine. They had scaled the top. Later on a piece of rock 10""WX10""LX3"" thick fell and hit him on the back as he was bent over.
The EE was helping put a tram motor that had de-railed back onto the track. They were using a lever hoist, air bags, and a lifting jack to hoist the motor back onto rails. The EE was assisting with the jack bar as another EE was jacking with the lift jack. The lever hoist slipped, shifting the weight back to the jack motor, motor landed on EE's right leg. EE able to pull leg out.
EE was helping position 12"" DR15.5 HDPE pipe for fusion. While positioning the pipe the stored enegry created during the pulling of the pipe result in the pipe suddenly being projected over the stationary pipeline towards the EE striking the EE in the face/head. EE was transported to ER he received stitches on his hairline to forehead and had a broken nose.
Employee was removing the front cover from a haul truck and fell hitting his left wrist on the ground causing a wrist fracture.
Employee strained his left elbow/forearm while attempting to dispose of a 30 Gallon thrash bin from security gate.
While operating the continuous miner, a rock was thrown by the cutter striking the individual in the head above the right eye causing a contusion.
EE was working constructing cribs on top of a canopy. When he jumped down to get off the canopy he thought he had turned his ankle however when he was xrayed at the ER it showed to be fractured.
Employee was helping 2 other employees install a tailgate and was positioned in the center. Employee positioned his hand on the lip of the tailgate to get ready to lift it up, once he started to lift, he felt a sharp pain in his right hand and then released the tailgate.
Employee stated that he was coming down the steps on the loader when his foot slipped off the step. He grabbed hold of the hand hold on the side of the engine compartment to prevent him from falling. As a result, he pulled a muscle in his left shoulder.
The employee was outside of a Terex Haul Truck when he reached back in the cab and activated the Engine Compartment Suppresion system for the truck. The employee then inhaled the dry chemical extinguishing agent that was released. Employee complained of shortness of breath and was taken for medical attention. Employee was released to return to work on 10/29/10.
The employee was mopping the floor of the gate house. During which time he pivoted on his right leg twisting his knee.
Heavy equipment operator was driving excavator to top of ""muck"" pile (muddy organic material remaining after deforestation of a swamp). The excavator's tracks unearthed a 15' oak log approximately 10"" in diameter causing it to enter the cab of the machine through the lower front windshield. The operator received a fractured left ankle.
Employee picked up a rock that was in the mine traffic area and strained his back.
WHILE BOLTING A PIECE OF ROCK FELL BETWEEN THE PINS STRIKING THE TOP OF HIS RIGHT FOOT.
EE was in the number 4R crosscut hanging flypads with co-worker. He bent over and cut the flypad 3/4 way through & stretched to finish the cut. The knife slipped and cut his left forearm. He was treated by EMTs and transported to Charleston Area Medical Center. He received eight stitches. He worked until 10/27/10 restricted until no work available as light duty.
While walking to get bolts to supply bolter, employee stepped in a hole, twisting his knee. This accident was not reported to us until 10/27/2010 and he did not go to the Doctor until 11/1/2010. He remaind at work until then. He is currently working light duty on the surface.
While chopping a 4 inch thick slab 4' long employee tried to keep it on the table but could not do so. He tried to get away from it as it rolled off table and it landed on right foot.
The injured was lifting a miner cable and felt pain in his left elbow. The injured continued work until after medical exam determined he had epicondylitis of the left elbow this injury is being reported as an occupational illness. His first missed day from work was 11/3/10.
Was dismounting a Euclid truck utilizing the steps and hand rails when his right foot touched the ground his ankle became twisted causing a twisted ankle sprain.
The medical was reviewed and it was made known on 1-17-11 that it was work related. Physical Therapy was ordered and a series was completed. The employee was driving his dozer, backed into a feeder and was jostled, resutling in an injury to his shoulder.
Employee was walking around a pallet. The shrink wrap on the pallet had come loose & the end of the wrap was on the floor. The employee did not see it & tripped over it. The employee's knee landed on the forks of the forklift. The employee continued to work with no problems until 12/9/10. He then requested to see a doctor.
The man was riding on a personnel carrier as a passenger. As they approached a run through check, the man raised his foot to deflect the check. The man's foot contacted the roof causing his leg to jam and twist his knee.
Employee was mixing Nordbak to put in between liners. Mixer was raised up high enough that it spun out and up into his face under his glasses and into right eye.
Employee was coming out of the bathroom and stepped wrong on steps and sprained ankle. Note: At first it was thought to be sprained, several days later learned it was fractured.
Employee was in the pit building a water stand. Employee was flipping an I-beam over. When he picked up the beam, something popped in his left index finger. Received a sprain to his left index finger. Employee is restricted and is able to work while restricted.
Employee stated that he was working on the pilot control valve on a D11R dozer. He hit his left knee against the hydraulic control valve, causing bruising and swelling of left knee. Reportable when he went to doctor on 10/21/2010 and was put on restrictions until follow-up appointment.
While moving equipment/supplies from the underground drill doctor shop to the new underground drill doctor shop, the miner strained his lower back muscles.
Employee was checking for grease leaks around the swing shafts in the dragline revolving frame. While he was in #1 swing shaft room he stepped on the sloping floor near the base of the swing shaft. This action caused him to turned his right ankle inward causing him to fall to the floor. When he stood up and transferred weight to his right foot he heard a pop and felt extreme pain.
steer cart out of elevator onto drive floor of #1 conveyor. Cart had a motor for #1 conveyor weights 6180 lbs wheels of cart grabbed door channel on elevator throwing employee to floor. Employee sought medical care on 10/20 return to work normal activites. Subsequent doctor visit on 10/25 states employee off work beginning 10/24/10.
Injured employee and another worker just finished repairing a damaged belt. Injured employee was picking up extension cords to take back to the tool trailer. When he started down the stairs he slipped and fell resulting in a ligament sprain to the right foot. The steps were not wet or slippery and there was nothing on the steps creating a tripping hazard.
Injured was instructed to pour grinding aid into the feed chute on #6 finish mill. He poured it thru an access door. After pouring material into chute, he closed door and a flashback occurred. Inujured suffered 2nd degree burns on right side of neck and right arm. He also suffered a fractured little finger on right hand.
EE was on the back of Red Top and was walking around the two Miller welding machines and lost his balance and fell to the ground. EE fractured his right upper arm between the elbow and the shoulder and chipped a fragment of bone in the same area.
While changing a blank in the cement conveyance line, the EE felt discomfort in his lower back. The EE seeked medical relief 2 days later and was placed in physical therapy.
Employee was helping install sanding unit on small dump truck, felt sharp pain in lower back. Employee saw chiropractor, employee said he couldn't work-no documentation available at this time. On 10-25-2010 chiropractor said employee could work with 20 pound lifting restriction.
Employee was bending a 5ft roof bolt and felt pain in his left testicle/groin.
The employee was operating a jackleg, while inserting a split set bolt. The pivot point gave way, pushing employees right hand smashing the fingers between the cob and rib.
While lowering middle belly pan on dozer, sling slipped and pan fell against right arm causing laceration.
The employee was hanging a continuous miner cable and strained his back.
Employee was fueling a bulldozer when he fell (approx. 3 feet) to the ground.
EMPLOYEE HAD PULLED UP TO MINER WITH HIS SHUTTLE CAR AND HAD JUST SWITCHED SEATS WHEN THE MINER OPERATOR SWUNG THE TAIL OF THE MINER AND BUMPED THE CAR INTO THE RIB. THE CAR OPERATOR'S THUMB WAS BETWEEN KITCHEN AND RIB. HIS THUMB WAS BROKE AND A TENDON WAS CUT.
Employee was roof bolting at the Face, he put first steel in roof when roof rock (rock 18"" x 8"" thick) fell when he was reaching for steels, rock struck his right hand little finger.
Mechanics were removing the impeller from a cyclone feed pump. A mechanic placed his hand on the belt and his right thumb got caught between the drive belt and the motor sheave resulting in an open frature.
Miner was stepping off of mantrip using three point contact and while planting his right leg, EE aggravated an old injury by twisting his knee.
The investigation into the accident is on-going, but as of now, it appears as the three employees were installing ground vault indicator lights when an ach flash occurred, injuring them. The employees were transported to the hospital.
Lifting wooden concrete forms approximately 25 lbs.
The injured was operating #85 Damascus, battery operated 2 man vehicle. He and a passenger were traveling down the South Slope. The vehicle struck the corner of the #9 shelter hole causing the injured to strike the steering wheel suffering injuries to his chest. He was transported by ambulance to Good Samaritans hospital, and later transferred to Deaconess.
Employee was tightening bolts on the Joy miner and felt something pop in his arm, shoulder & elbow.
(The shift started at 6:00PM on 10/6/10.) EE was dismounting the water truck & stepped on a rock with his left foot. He lost footing with the left foot & his right foot slipped through the ladder rung, becoming stuck & twisting at the knee. He heard a ""pop"" & felt pain in his knee. An investigation of this incident was completed on 10/20/10, at which time it is being reported.
The EE is regularly a truck driver. On this day he was filling in to help the mechanic repair a piece of equipment. The EE was wearing required protective gear. He was laying on the ground to brace the ripper, they were attempting to put a pin in, a metal chip flew and pierced his right forearm. He was unaware of the injury until he saw the blood.
Crew was recovering track rail on #2 section. They were using a scoop and chain hoist to lift the rail onto the rail car. One rail caught the car and jumped the upright standard (8""). EE turned to get out of the way. The rail caught the calf of his right leg. Tests were negative. Worked restricted until 10/27/10 and then off for surgery to repair muscle.
Installing a new dryer controller, the controller indicated a flame in the dryer. Employee confirmed the presence of flame in the dryer. While attempting to close the main propane supply valve, the valve cracked and broke. Propane escaped and was ignited by the flame in the dryer. 1st and 2nd degree burns to arms and face.
Employee was using a pressure hose cleaning around the wash plant. He was washing around the log and was in a tight area. He felt his back strain.
Employee was on conveyor cutting belt with a chop saw when he slipped and the chop saw fell on his left hand. Employee received surgery to repair tendons in hand.
Employee was pulling the truss gun off the whip while standing on top of the bolter on 8 foot bolts. The bolts rolled, causing employee to fall. He reached out to try to break his fall and heard his shoulder pop. Employee became a lost time injury on Nov 11, due to surgery.
Coming out of excavator, slipped and hurt his knee. He did not report it till 12:45 PM. Another EE stated he stumbled on cable and fell and twisted his knee.
EE was working inside the #1 scalping screen, upon exiting the screen, he twisted his left knee while trying to make it from the discharge lip of the screen over to the crusher chute then onto the breaker building floor.
While pulling old dust collector bags off cages in main baghouse, employee felt strain in his shoulder. (Did not become reportable until employee started to miss work on 1/10/2011 due to surgery).
Employee was moving Angle Iron into warehouse, piece slipped out of his hands and bounced off his leg. Even though this occurred on 10/6/10 he did not report that he needed to go to the doctor till 10/14/10. The injury occured on the last day of his scheduled 7 day rotation. He did not seek medical attention for this until the last day of his 7 off 10/14/10.
Employee was exiting scoop deck when he slipped and twisted his knee. He was placed on restricted duty on 10/21/10.
