Fall was found on 11/30/10 MSHA was present and notified about roof fall. It was located #1 entry between bk 12&13 on the 4th west mains. Area will be dangered off, fall will not be cleaned up MSHA has completed their investigation of the area.
Roof fall found while traveling old works on 1st West Mains.
Due to an electrical storm in the area, the mine site (along with others in the area) lost electrical power. Mine evacuated via auxiliary power source. No injuries or other problems. Power restored at 7:00am. THERE WAS NO DAMAGE TO THE HOISTING EQUIPMENT - PRODUCTION OR MAN HOIST!
IGNITION: The Longwall Shearer was cutting bottom with the Headgate drum when an ignition occurred. It occurred at approximately No. 115 Shield on the H/G Drum. The fire was around the drum and the flame was put out with the drum sprays. No other fire extinguishers or water was used. The fire lasted only a few seconds and went out on it's own.
At 9:01am the elevator left the bottom landing area with the door not completly closed. The elevator was removed from service until repairs could be made. At 10:10am the door switch was replaced and the elevator put back in service.
The man and material shaft hoist faulted as it was lowering the cage into the mine. Approximately 20 persons were on board and were unable to get off of the cage until around 12:25 PM. The hoist faulted due to motor cooling fan blades coming loose and falling into the motor. The hoist returned to operation on 12/2/10.
Operator was pushing with a Cat D9 dozer when smoke started from the engine compartment. Operator turned off machine, called for assistance from foreman. Both tried to extinguish the fire but could not. Local fire department called to extinguish fire.
An unintentional fall occurred between the #4 & #5 entrys of the Main North entries at the 55+40'. The fall was appoximately 17W X 17L by 8' high. The fall occurred do to deteriation of the roof above the bolt anchorage.
A roof fall was discovered on A Sub-Belt (#27 Block, #3 Entry). The fall was 18' x 10' x 80'. The area was bolted with 6' SuperTwist and 12' cable bolts. No people, equipment, or ventilation was affected.
A roof fall was discovered at spad 310 in the #3 entry at 9:55 pm during a preshift examination. The fall was appx. 20 feet long x 16 feet wide x 8 feet high. The fall did not affect miners,ventilation nor equipment.
We had an unintentional roof fall in #3 entry XC 23 in 3rd West.
At 9:00 pm a roof fall was discovered by an examiner on the Main North Belt Line at XC 38. There were no injuries or witnesses. Also no disruptions to ventilation or passage of personnel.

Moved North skip to surface for Chippy test trip. When beginning Test trip for #1 hoist would not reset.
A roof fall was encountered on the weekly examination of the Parallel Mains. The roof fall is located at 34 break, near spad 20786. Travel/ventilation was not impaired. The approximate size is 18'wide X 20'long X 4'high. The area will be breakered off.
The Livingston Portal elevator stopped operating when the elevator car stopped below the mine level landing. The elevator repair service responded and placed the elevator back in service at 10:45 AM. There were no persons injured, or entrapped, as a result of this incident.
Full investigation was sent to field office. We had a fire in the kiln coal mill. The CO2 suppression system activated. All was well until 14:06 when CO rose quickly in the dust collector. The concern was fire. We manually activated the CO2 fire suppression system and the CO went to ""0"". No injury. No property damage.
The Somerset Portal elevator stopped working while at the bottom landing. The doors would not close, and the elevator would not operate. The elevator repair service responded and had the elevator back in service at 8:25 AM. There were no persons injured, or entrapped, as a result of this incident.
Employee was driving by 1422 muck bay enroute out of the mine. At that time it was noted that a portion of the right rib starting at the right pillar on the edge of the L2W ramp and progressed toward the muck bay face. approx. tons from the failure is 50.
Matting timbers were used to support shovel side frame caught fire which lasted more than 30 min. Presumed to have been caused by slag from welding/cutting torch the previous afternoon. Work was completed at 3:00 pm 11/23, final walk around at 5:45 pm. At 4:30 am 11/24, fire noticed by night shift personnel. MSHA notified at 5:02.
#3 Hoist East Brake would not release as the brake line was frozen. There were still several escapeways to surface available for personnel underground.
Ground fall occured approx. 50 feet back from face. it failed above long support in between a few smooth walls and the paste fill. The ground fall was approx. 25 feet long. Tapering upwards to about 15 feet high and about 20 feet wide. Est. approx 150 to 200 tons. Did not impede ventilation or escapeways.
An unplanned fall of the mine roof occurred above the anchorage zone on the 1 South Mains section in the #2 Heading, at an overcast for the 14 Left belt trench. The fall measured 22' long X 8' wide X 6.5' high.
The emergencey stop switch shorted out on bottom, had to trouble shoot the problem, then replace it.

A roof fall was found at approximately 9:15 a.m. on 11/23/2010 in the No. 2 Entry of B-Intake. The fall was approximately 20 ft.long x 18 ft. wide x 5 1/2 ft. high. Five foot fully grouted roof bolts were in use in the area.
Weekly examiner found an unintentional roof fall between the #9 and #10 crosscuts in the #3 entry of the 9F section. The fall measure approximately 60' long by 18' wide by 8' high.
During a weekly examination of our M3 Main Returns a roof fall was found at x-cut 26, entry #1. The fall was found in the intersection only. It measured 20' long, 18' wide, and 15' high. the fall did not cause personnel injury to miners, impede passage of miners or affect ventilation.
An unplanned roof fall has occurred in a worked out area of the Appaloosa Mains near spads 4750 to 4620. This area is used to evaluate EP8 and EP7. The roof fall was 800' x 20' x 4'. No injuries or equipment was involved.
A roof fall was discovered in the 3L panel xc-17 #9 entry at spad #11+05. The fall was length - 17ft, width - 19.25ft, and height 6ft plus.
#3 production hoist was down due to a vibration trip. Two separate escapeways to surface were still available to personnel underground.
Employee entered lube room of the #575 electric shovel (P&H 2300) around 7:30pm. The door closed behind him. The latch was dirty and would not open from inside.
An unplanned roof fall occurred in the return entry 2400' from the active face, inby survey spad #34863. The fall was approximately 20'x20'x8'. There were no injuries or equipment involved in the fall.
Unplanned roof fall occurred in North East Main Panel at BK#10 in the #7 entry spad #239. Fall was approx. 112' L. X 19' W. X 12-14' H.Date of development was 10/07. No injuries, entrapment or equipment involved.
At approximately 3:00 pm an ignition occurred on the J- main section in the #3 entry. The roof bolter operator was re-tighting a rib bolt when an ignition occurred around the rib bolt. The flames were extinguished in approximately 10 seconds with rock dust.
A roof fall occurred in the D-mains travel way #5 entry (extra intake). The fall measured 20'wide, 40'long, 5'high and supports with 4' fully grouted resin bolts.
Emery Mine experienced an intentional fall in the pillar line (GOB) on 11/22/2010 MSHA has requested that we file a 7000-1 report for this incident, we do not agree that this is a reportable accident or immediately reportable.
An unplanned roof fall was discovered by the weekly examiner 2100 ft outby the #2 section right return #7 return near spad 5753. Fall measured 60' long by 19' wide by 12' high. This area will not be cleaned up.
A fireboss during a preshift examination found a roof fall measuring 100' L X 20' W X 6' H in the 5th West 8th South connection # 1 x-cut # 3 entry. The roof fall did not effect escapeways or air currents. The area was bolted with 5' combination bolts.
Unintentional roof fall on the 8Q longwall from the face to the tailpiece. The fall measured approximately 85' long by 18' wide by 12' high.
There was an unintentional fall of the roof on #2 unit's 3rd beltline between crosscut 9 and crosscut 10. The fall measured approximately 50' in length, 17.5' wide, and approximately 10' tall.
A fall occured in the unsupported face area of the #6 Entry, 1 break inby spad #775, 5 ft thick, 10 ft wide, 15 ft long. The unsupported fall did damage 4 bolts in the last two rows.
IGNITION: Employee was on his knee over a gas pocket cutting a roofbolt to make a punch out. Was on the second cut when it lit up, got up and stomped the fire out. It burned for about 10 seconds.
Hot work activity was conducted at the Solid Fuel AFR system which likely caused a ignition point which caused a fire on the V93-BC2 transfer belt. Fire caused approximately $30,000 in damages to belt transfer systems.
The service elevator went down due to a stuck contactor. No persons on board at 2:05 p.m. Elevator repaired at 3:07 p.m. Repaired contactor.
An unintentional roof fall occured in the intake entry outby the #3 working section. The fall occured due to the fact that the roof strata broke at and above the anchor point, exposing a slip in the fall area. The fall will not be cleaned up, the fall area will be timbered off and all approaches will be dangered off.
Intermittent problems with the hoist's brake caused the hoist to be shut down for a repair. The three-way valve on the SOBO Unit was replaced.
A roof fall was discovered at 5 south #2 entry, # 6 cross cut. Fall measured 15' W. X 15' L. and was above the anchorage.
A roof fall was found today that was a continuation of a roof fall that had occurred on March 29th, 2010. The fall is located at #7201 spad in #5 entry & goes to #7202 spad in #6 entry. It ties into the fall at #7147 spad. Fall is approx. 75' long x 20'w x 16' high. This is on a panel that is not being mined. Area timbered off & dangered off. No injuries resulted in this.
While night shift was beginning their shift, a SMD contract employee discovered one of SMD's haul trucks burning at the 4510-519 stope. Attempts were made to extinguish the flames, but were unsuccessful so the mine was evacuated. All personnel were evacuated with no injuries and the brass board was cleared at 7:45 pm. Several mine rescue teams were utilized to put the fire out.
A roof fall was found in 1st left at break 2 and 3 in the #4 entry at Double Submains, the size of the fall was approx. 20'W x 80'L x 6""thick. area of the fall has been cribbed off.
Foreman was traveling with MSHA inspector when roof fall was discovered in #3 entry 5 break of North Parrallel Mains intake near spad 21261. The intersection is being timbered off being there are other entries to travel and it was not a primary travel route.
Fall was found at 1st left off 1st left off 2 south mains 61/2 break to 10 break #2 entry near spad 1202. Fall was 160' long x 19' wide x 8' thick. Roof support was 6' torque tension bolts.
An unintentional roof fall was discovered in the #2 entry (return) at #36 break spad #676 adjacent to the 1A belt. Size of fall was estimated to be 30'L. x 19'W. x 4-5'H. Fall area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught. Ventilation was not interrupted. Fall will not be cleaned up.
A rockburst occurred during the dayshift blasting time. It caused a section of the back in 54 ramp to come out. the section was approximately 6' high x 14' long x 8' wide. It was blocking the roadway in the drift.
Unplanned roof fall occurred in Sub-Mains ""A"" panel off of 1st NW panel BK #4. Fall was approximately 25' in length x 19' wide x 14' to 16' high. Date of development was May 2009. No injuries, entrapment or equipment involved.
Unintentional roof fall occurred in the #1 entry, break 69 on South Mains. The fall measured 8'Hx18'Wx50'L.
A roof fall was found on 11/16/2010. The fall measured 20'x 20'x 3'. The fall was one break inby spad No.3460 No.7 Entry. After finding the roof fall, all accessble areas was timbered off and visible danger signs where posted.

Fall was found at 1st left off 2 south mains at 1 crosscut inby spad 1058 to 1055 no.2 intersection. Roof support was 6' torque tension bolts.Fall was 100' long x 20' wide.
An unintentional roof fall occurred at the 5th north panel off of the 1st west panel off the north mains. The fall measured 25 feet x 18 feet x 6 feet. The fall was in unit 2 return. The fall area had been roof bolted with 6 foot fully grouted roof bolts. The fall maybe cleaned up at a later date.
A fall occurred on the B-2 Panel (#21 Crosscut, #3 Intersection). The fall area was 18' x 18' x 18'. The area was bolted with 8' SuperTwist bolts and 16' cable bolts. No equipment, travelways, or employees were affected.
Roof Fall 3rd west submains 1 bk outby spad number 31423 goes into #2 entry fall is 6' high x 19'w x 300' long area is dangered off and checked by MSHA during routine inspection and will not be cleaned up.
Found an unintentional roof fall in #5 entry of #16 return at break #52. Fall is approx. 12' long x 20 wide x 8' high.
As the 5500 excavator was pulling overburden there was an unplanned detonation of an explosive material. There were no injuries. There were no damages to the equipment.
An unintentional roof fall occurred at the New Future Mine 1W tailgate, entry #2 crosscut 16 2127 W. The fall was approximately 25 feet long, approximately 17.5 wide and approximately 10 feet thick. The fall will be cleaned up fall #27C
The lime transferring system shutdown, plant personnel tried to reroute the lime. The hot lime sitting on the conveyor caused the belt to start on fire. Plant personnel tried to extinquish fire and call the fire department at 18:06. Plant management was contacted at 18:53, came to the plant to assess the situation and contact MSHA at approximately 19:00. Fire was out around 21:00.
A roof fall occurred on the old 4 west mains at 23 block cross cut 8 to 9. The fall did not disrupt ventilation of the walk way. No people were affected by the fall. The fall area was dangered off and the brawls supported.
An unplanned roof fall occurred 1 X-cut outby spad #46558. The fall was about 16' wide, 20' long and 5-6' high. The area was bolted with 5' torque tension bolts. The fall will be cleaned up and rebolted.

An unintentional fall of the roof occurred in the intersection of the longwall face and the tail entry at cross cut #96. The length of the fall is unknown and this fall will not be cleaned up.
An unintentional roof fall occurred at the New Future mine 6 seam west mains entry #2 around 1920 W. the fall was approximately 40-50 feet long, approximately 18.75 feet wide and approximately 20 feet thick. The fall did not affect passage of men. The fall will be cleaned up.
At approx 2:00PM, a roof fall occurred on the #7 beltline. The fall was 100' in length and ranged from 7-15' high. The fall was located between walls 69 and 71 and adjacent to wall 70 along the 1st Northeast Mains.
An uplanned fire occurred at Imperial on the surface when a contractor owned front end loader caught fire in the cab of the machine. The fire was put out by mine personnel using fire extinguishers. Fire departments responded to the scene. The loader was not in use at the time of the fire.
Roof fall appears to have originated at or near the face in heading 1 & extended approx 100 to 120' outby. It is approx 30-35' in width at its widest point & extends above the roof line approx 15-19'. It is believed that a combination of low cover, lower structural quality of roof rock & possible localized horizontal stress had created the conditions leading to the failure.
Fall discovered in outby entry - approximately 4' thick x 50-60' long x 18' wide approximately 3000 ft outby 2 section between #7 - 8 entry.
The mine experienced a roof fall measuring approximately 18 feet by 70 feet by 8 feet in the worked out area of the 4 right panel. There was no interuption in the ventilation.

The date of development was November 2008. No injuries, entrapment or equipment involved. Stoppings will be re-routed to re-establish the right return air course.
Roof fall 6 South Rooms (intake) 2 Room at 1+50 intersection adjacent to 19+28 F Entry. 17 ft wide, 20 ft long, 6-7 ft high.
As the 3600 excavator was pulling some wall down there was an unplanned detonation of an explosive material. There were no injuries. The damages were only to the windshield and paint of the excavator. This event is still under investigation.
A roof fall of unknown length x 20' x 3' to 5' thick occured in the #1 return entry of 3 East Mains, 90' inby spad 3251 and 1150' from the active section. All access to the fall area has been dangered off.
A roof fall was discovered at the 3rd Northwest submain #6entry,xc-74 and spad #60+30. The size of the fall was length-20ft,width-19ft and height-8ft.
A roof fall occurred in the #6 entry spad # +3796 of the 1st NE main. This fall was located in a cross-cut adjacent to the travel road. This fall was located approx 15,000 feet from any working section. Size 19' by 35' by 6 plus feet high.
A roof fall 30' x 20' x 5' thick occured in the #2 return entry of 3 East Mains, one break from spad 3263 and 1050' from active section. Timbers were set in the area prior to the fall. The return travel way has been re-established.
A non-injury roof fall accident has occured in the no.1 entry of the left return on panel 32 off the 4th northwest mains. The fall wa discovered during the weekly exam of the left return airway. The fall is located approximately 50 feet inby survey station no,1098 in the intersection. the fall is approximately 40 feet in length and approximately 6 feet thick.
A fall of roof was discovered in the 3L Panel in the #4 Entry at cross cut #13. The fall was approximately 20ft long x 28ft wide x 8ft high.
An unintentional roof fall has occurred at the #17xc of the 2nd Main south between the number 2 and number 1 entries. The fall was approximately 35 feet in length and 17 feet wide by 8.6 feet high. The fall was in a cross-cut and both approaches to the cross-cut had been timbered.
Had an unintentional roof fall at break #22 on the #21 belt. Fall is 20' x 20' long x 10' high.
A fall of roof occurred in the Southwest Submain at crosscut # 56, entry # 3. The fall measured approximately 18 feet wide x 18 feet long and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
A fall of roof was discovered on in the 1st North West Mains in the #4 Entry at cross cut #37. The fall was approximately 70ft long x 19ft wide x 8ft high.
Rock fall occurred on #1 North East Mains 1st left on the 2nd panel in the #2 entry. Fall measured approximately 19' wide x 140' long x 6'-8' thick.
On the tailgate end of the G-9 Longwall, the rib above the last shield on the tailgate end rolled out. Although the rib shows some deterioration, the mine roof was never compromised and the tailgate gate entry was never blocked from passage. The operator implemented it block tailgate plan as a precautionary measure.
Slick-n-side formation 41/2' to 5' thick pulled 1 row of bolts and damaged a 2nd row in the primary escape way.
An unintentional roof fall was discovered by the mine examiner in panel off 2nd NW sub mains. Fall meas. 100' L. x 19' W. X 7' H. No equipment or miners were involved. Area will not be rehab'd. 1 X-C inby spad #5190.
A Cat D11R Bulldozer was found on fire after work hours by an EE who was coming to work on the next shift. 2nd shift had parked the Cat D11R Bulldozer at 2:00AM at the end of the shift in the parking lot. EE coming to work on day shift at 3:30AM came upon the fire as he came in to work. No one was injured. Cat D11R Dozer was totaled.
Examiner went into return of 1st West Mains on the morning of 11/5/10 and discovered a roof fall which impaired his normal route of travel.
An unintentional roof fall was discovered by the mine examiner in panel off 2nd NW sub mains. 2 x-cuts inby spad #5184. The fall measured 40' long X 19' wide X 7' high. Area will not be rehab.
A fall of the mine roof was discovered in the #6 entry at 100 brk of #2 south belt line. The fall measured approx. 200' long x 20' W x 6' T. Ventilation or travelways were not affected. The fall has been breakered off on all approaches.
Had a roof fall in #7 entry at brk 14 spad #73. Top is heavily laminated in area of fall. Additional cribs were built in affected area and surrounding areas. Area was completely dangered off. There were no injuries to report.
A roof fall measuring 120' x 19' x 10' thick occured at spad # 10952 in the return air course. The area was bolted with 6' tension rebar bolts.
A roof fall was discovered at the #4 Mains Section (#4 Heading, #20 Crosscut). The fall measured 16' x 16' x 8'. The area was bolted with 6' SuperTwist bolts and 12' cable bolts. No equipment was damaged. No ventilaitn was disrupted, but the intake travelway was impeded for a brief time. No one was injured as a result of the roof fall.

A roof fall has occurred in the No 5 entry, just inby survey station spad No 3185 of the No 2 section area of the mine, in the right return, the area measured approximately 25 feet long,19 feet wide and 5 foot high. The fall did not entrap men or impede travel. The area will be cribbed and dangered off and shown on the mine map.
A fire involving an oil well treater inside the permit boundary was discovered at 3:03 pm. At no time were mine personnel or mine facilities threatened. The cause of the fire has not been determined. It is suspected that the age of the unit is a factor. The fire was extinguished at 4:00 PM.
There was a methane ignition in the #2 entry of the 12 East section.
A roof fall was discovered on the 4 North Return at Crosscut #37 in the #1 Entry. The fall measured 30' long by 20' wide and was approximately 7' in thickness. The area was originally supported with 5' Hy-Tech Bolts. The area was dangered off.
A roof fall occured in the #4, 2nd East Panel. The fall is 180 feet long, 20 feet wide and 10-15 feet high.
Dust collector duct to crusher dump pocket caught on fire during dust collector demolition
An area of broken up slate stone approx. 10 feet in width and 8 feet in length and approx. 7 feet high fell. This area was bolted according to the plan. This fall was cleaned up and the mine roof was re-supported. There were no injuries, equipment, or entrapment. This fall was immediately reported to MSHA. MSHA representative was on site and inspected this roof fall.
Roof fall was found off of 1 west mains 2 left panel #1 entry. Roof fall was approx. 55' long x 19' wide x 7' thick.
Roof Fall in E- Butt Return area , fall located at at spad 3469 approx.30 ft outby
A roof fall occurred in the 2nd Southwest Mains, crosscut #34 in the intersection of #7 entry. The fall measured approximately 19' x 19' x 10'. The area was supported with 6' tension rebar roof bolts and 8' double lock mega bolts. An undetected slip with the presence of water was determined to be the cause of the fall.
On 1/1/10 an unintentional roof fall occurred on 002 Pillar section inby spad #9888. This fall caught the #3 M.R.S. Fall was 5'X15'wX15'l. MSHA issued K order which was modified to allow cleanup and t recover M.R.S. Area was bolted with 10' cable bolts. M.R.S recovered at 7:30am on 11/2/10 and K order was terminated. Area was dangered off and timbered
At approx. 7:44 a.m. the C.O. room operator recieved a phone call from underground stating that the I-O section belt line was on fire. At approx. 10:10 a.m. the belt line was reported out.
While exploring a mine that had temproarily abandoned and the fan restarted under a exploration plan 4 unplaned roof falls were discovered. At spad #37: Fall measured 16' x 16' x 4'
An unintentional roof fall occurred in the #1 entry inby the last open break in the face. Estimated size of fall 40'L x 18'H x 4'-5'. Area had been supported with 4 foot fully grouted resin bolts, no injuries, no equipment caught, ventilation not interrupted. Fall will be cleaned up/or dangered off and developed around the fall area.
The head of the 13B conveyor caught fire, causing the belt to catch fire. The fire is under investigation
A roof fall was found in the #4 entry of Plant Mains, at Brk.10 from the outside, the fall was approx. 5'thick, 20'wide, 20'long. Ventilation or escapeway was not effected. The area will be timbered and dangered off.
A x-cut fell 17'wide X 46'long X 6'high. It was already cribbed on each end. The fall was examined and dangered off and reported. No further cribbing needed. It will not be cleaned up. It was in #7 to #8 x-cut near spad #2049 and #2050. It was in an outby return.
Maintenance was being performed on 10 wheel truck. It appears that the chain slipped causing the truck to fall striking the victim. The victim died from possible internal injuries. Still awaiting Coroner's and MSHA's finding to complete the investigation.
A Mack DM-600 tandem axle lube truck was descending a grade into the active work area of the mine when the operator lost control of the truck and either jumped or was thrown from the vehicle. The truck struck an embankment and overturned on its left side resulting in fatal crushing injuries to the victim. He was pronounced dead at the scene by the coroner.
The contractor failed to use emergency brake, left tractor running and went under tractor to look at leak. The tractor rolled due to being on incline and the result was a fatal injury.
EMPLOYEE STATES HE WAS REPAIRING ACTUATOR ON DUST SYSTEM WHEN ACTUATOR BECAME ACTIVATED, CATCHING HIS FINGERS, RESULTING IN A PARTIAL AMPUTATION OF LEFT MIDDLE FINGER AND FRACTURE OF RING FINGER.
Employee was replacing the bit on the Ingersoll-Rand T4 drill. He was using his left hand to steady the hammer while he reached inside the cab with his right hand to operate the control to lower the hammer onto the bit. As the hammer was being lowered, it moved downward suddenly pinching the employee's left thumb and index finger between the hammer and the bit.
Employee was checking to see what was wrong with head on roof bolter. EE asked helper to raise bolter head so that he could look, unaware of having placed left hand at pinching point, resulting in mashing ring finger tip off and cutting middle finger below nail bed, later having to remove middle finger below nail bed.
Contractor cutting bottom of hopper. Hopper was secured with rigging. As cut was complete, hopper shifted two-inches pinching right hand, amputating index finger distal section.
EE was working on a truck that wouldn't start; he reached in to rotate the crank shaft by pulling on the fan belt when another employee activated the start switch. His fingers were caught between the belt and the alternator pulley. The ends of his little and ring fingers on his right hand were amputated.
The employee reached for a pry bar to help align the cylinder with the flange hole for placement of a pin. He took his eyes off his left hand which was on the side of the cylinder. His hand moved so his little finger was now resting on top of the cylinder. It is believed that the cylinder was bound up and it dislodged moving up approx. 1/4"" pinching the employee's finger.
Moving a welding machine using a forklift. He stopped the forklift, set the parking brake, and turned it off after he lowered the welding machine onto the ground. As he was unhooking a stabilizer chain the welder shifted on the ground pulling the chain and the tip of his index finger was mashed between the chain and the forklift mast. Was wearing leather gloves.
Employee felt a sharp pain in the Groin area while he was working to remove a metal rod from a mobile crusher. Hernia
EE had gotten his fingers wedged in between the sleeve and arm on the #2 Roaster on 8th hearth from south west man lift. Two other employees were pushing on sleeve attempting to get the arm in the sleeve when EE's fingers got pinched, causing his fingers to be cut.
EE was standing at equipment when he blacked out. Fellow EE found him within 30 mins. He was dazed and disorientated. EE went to E.R. where he was examined and nothing was found.
EE was assisting a co-worker to attach a belt tool trailer to a battery operated ride at break No. 41 on No. 7 roadway. While co-worker was moving the ride, EE was struck by the ride resulting in contusion of both legs.
EE was rib bolting and had to reach over the drill pot to insert glue and a bolt. While doing so, EE experienced a burning sensation in his lower back.
The ee was experiencing pain in neck and elbow when handling miner cable 4-section #3 entry CC outby face in intersection on 11/30/10. Medical received 12/8/10 shows there was no acute injuries of neck, shoulder, arm or elbow. However other medical shows other issues exist causing pain to the affected areas.
EE was carrying an extension ladder up a flight of stairs with another EE when he felt a pop in his knee.
EE was dragging a 1"" water line up the ramp on the Raw Coal #2 belt, when he felt pain in his right groin area. He ws diag. with an inguinal hernia and was removed from work on 12/2/10.
Employee was ripping with a dozer and ripper shank broke jarring employee. This became a reportable on 2/3/2011 when employee received a cortisone injection from Dr.
Maintenance was being performed on 10 wheel truck. It appears that the chain slipped causing the truck to fall. The truck's tail light apparatus struck the worker on top of the right side of the head causing a laceration, a contusion on the left knee, and a sprained right ankle.
Employee was climbing down the ladder of a CAT 777 rock truck when his foot slipped on ice on the last step. The employee did have three points of contact and when his foot slipped off the step he pulled his left shoulder. PBS Coals, Inc. was notified that the employee will need surgery to repair his left shoulder. First day of work missed was December 21, 2010.
Employee was trying to turn around a lizard truck in a confined location on the bench while loaded with earthen material and hit a berm causing member to suffer from whip lash.
EE was working on a pump. When he stepped on a concrete brace with his inner foot he felt a severe pain in his left knee and noticed unusual swelling. EE was then transferred to a local ER for further tests/treatment. EE was taken off work on 11/30/2010.
Employee stepped into area to place bulk bag straps onto forks so bag could be lifted. Forklift operator drove onto his right foot.
EE parked 4-wheel battery buggy in high to load rock dust on it. Left directional contactor in reverse. Got against foot pedal, buggy went backwards and dragged EE into 30"" low top, mashing his chest against foot pedal and brake pedal. EE complained of very bad pain in chest and right arm.
Employee was helping lift a 60 H.P. deep well pump to sit on a wooden block when he felt pain in his groin area.
Employee was doing demolition of conduit and noticed a gradual onset of low back pain.
Lifting belt stand to place a crib under to line belt. The stand was stuck in mud. Hurt back.
Employee slipped on a set of steps but did not fall. He began to complain with lower back pain. On 12/9/2010 he was seen by a specialist and released back to work with no injury listed. The em ployee did not return to work and sought a second and third opinion and was placed off work by a clinic doctor. Employee did not have contact with employer until late April 2011.
Preshifting the life shelter was crossing belt back to buggy and slipped on waterline twisting left knee.
A mechanic was removing an old U-joint from a coal truck. In the process of striking the object to punch the U-joint out, the mechanic delivered a glancing blow, which caused a 6 inch long x 5/8 inch thick bolt to fly out and hit the injured party in the mouth. This caused a cut to the mouth, one tooth to be knocked out, one tooth broken & two teeth to be loosened.
Employee was attempting to bend bracket above conveyor belt by striking it with the sledge hammer. The hammer missed the bracket, struck roller, and bounced back, feeding it into the next roller. Employee was able to let go of the handle with his left hand but his right hand was pinched between the belt and the roller.
Employee missed rung when descending from ladder, injured wrist in attempt to catch himself.
The injured employee was adjusting a jack pipe for the tailpiece. In the process of doing this, the jack pipe fell and struck him on his back. This resulted in a fracture to a rib.
Employee was using a lady slipper bar and was jabbing it between the belt and metal platform and the lady slipper bar slipped thru with his left pinky going thru also. Was told not to by co-worker.
Employee was walking down steps behind shop. His foot slipped in the gravel when he stepped off the last step. Employee fell against a scoop mounted rock duster. He bruised his lower back.
While bolting top, he put his (L) hand on pot of bolting machine and crushed it with drill pot and top.
Employee was removing steel liners from the upper pan feeder discharge area. The liner to be removed was cut into smaller, more manageable pieces. When employee reached over a piece of liner that was resting against the handrail, the section of rail failed, causing the employee to fall.
Employee was taking down fiberglass vent tube and the vent tube bounced striking employee on the right knee. Employee continued to work until 1/28/11 and at that time, employee went off work to have knee scoped. Employee works 12 hour shifts and was off for seven workdays.
Operator of the 527 haul truck reported that he hit a pothole and had neck and back pain and trouble breathing.
Slipped on ice while getting into pick up, he grabbed door jam with arm and hands. Hurt his elbow. Slipped on ice 11/27/10, went to doctor 12/8/2010.
Employee was installing a track switch. Moved the bending jack and felt pain in left upper arm. Began losing time 12/6/10.
Climbing on drill, foot slipped off step causing left arm to catch his full body weight.
Employee was sowing grass with a hydroseeder hose when he pulled the hose and strained his lower back.
Employee was arranging the spreader cables on the back of the flatbed work truck when he lost his balance and fell from the truck to the ground. He says he is unsure what caused him to lose balance. Accident was not reportable at the time however 2/22/2011 it became reportable. The accident did not involve cfr 50.20-6 (i) (ii) (iii) or (iv).
Employee was holding a hydraulic hose on a continuous miner looking for a leak. He requested the pump motor be started. When it started another hose near his hand blew oil under pressure cutting his finger shooting oil into his hand. He was given 1st aid and continued to work. He began missing work on April 12, 2011 to have surgery for infection in his hand.
Worker was attempting to place a belly pan back on a Cat D-11 dozer, the pan was secured with chains on both sides, one chain shifted letting the pan shift backwards cutting the worker on the left ear lobe. Worker was taken for treatment which required 10-15 stitches and was released.
Employee was walking behind the surface maintenance shop when he slipped and fell on icy surface. **Management was unaware of the injury until employee's return after the scheduled shutdown for the holidays 12/22/2010 - 1/3/2011** Lost time as of 2/11 when he was scheduled to RTW, but had surgery to repair a torn ligament in his knee 2/4/11.
Employee was placing a 50 lb bag of dust on a pallet and while placing bag on pallet, his elbow popped.
On 11/23/2010 employee strained his back, shoulder and neck when he fell to the ground. Employee did not start missing time until 12/29/2010.
Employee was walking on snow covered ice while fueling the #2 excavator and fell forward to the ground. Employee was able to do the capacity of his job until March 4, 2011 when he was placed on restriction to office work.
Employee was removing melted snow and ice off of the walkway next to the truck scale. He lost his footing and fell impacting the ground on the right side of his body, resulting in a small closed fracture in the pelvic region. (Dr. said not serious.) Employee is moving around on crutches. (Employee was instructed not to shovel due to his age.)
Employee entered the closed guarded area of Blender 1 while the blender was in operation and his clothing was caught on the blender and he was thrown to the ground. He broke his left arm in attempting to cushion his fall.
Employees was installing forks on a Lo-Trac and fork fell hitting top of left foot. Employee did not start missing time until 11/29/2010.
EMPLOYEE HAD BEEN BEATING ON AN AREA THAT WAS WELDED ON A RAMCAR AND A PIECE OF SLAG CAME OFF AND WENT INTO HIS ARM. THIS HAPPENED ON 11/22/10. EMPLOYEE WORKED UNTIL IT WAS TAKEN OUT ON 1/21/11. SLAG WAS REMOVED AND HE RECEIVED 6 STITCHES, AND WAS TAKEN OFF WORK UNITL 1/31/11.
Employee shoveling coal when he turned & felt sharp pain in lower back.
Employee stated that he was laying on the frame of a 992G loader in an awkward position trying to change out two starters. He felt a pull/strain in right hip area. Did not bother him until the next night when he experienced pain in hip area. He went to doctor on 11/24/10 and incurred lost work shift on that day.

EE was using a pry-bar to separate a steel beam that was partially cut in two with an oxygen and acetylene torch. When prying with the bar, he felt a pain in his left shoulder. This foreman was taken off work on 12/14/10
Employee stated he was riding out of the mine in an open mantrip, tilted his head back to take a drink from a water bottle when the bottle struck the roof and shoved his safety glasses back into his forehead causing a laceration.
Employee was tramming a road grader, when he struck a header with the top of his hard hat. He drove to the 38 level and then went to medic on site. He had headache & was sore. He was taken to doctor, and was given some days off.
Climbing ladder to enter cab of haul truck, foot slipped off rung.
Working with bags from bag house inhaled lime dust.
Employee was removing engine block heater from John Deere 624-J loader while standing over the engine compartment. Employee started to slip and while preventing self from falling on the ground she strained her right arm, shoulder, and neck. Employee was seen in the emergency room where it was determined there were no broken bones only a strain.
Working on rock truck standing on step on fender, tightening bolts on muffler. Lost footing and came down on left knee. Not sure what he hit it on.
Employee stated he had an injury, that allegedly happened on 11/19/10. We were notified at 4:20 pm in a phone conversation on 12/07/2010. Employee has been off work since 11/23/10 on a non-work related health issue, results from some of his testing while in the hospital indicated 2 fractures in his vertebra, 1 old and 1 recent? Using a sledge hammer tapping grates into place?
While cleaning front of windshield on a Volvo 220 loader, ee got dizzy and fell off the loader to the ground. EE was taken to our local medical office and was told he had a broken foot.
On Friday, November 19, 2010 at about 11:00 a.m. the haul truck driver lifted a 35 pound wheel chock to replace it on the front of a Cat 785C haul truck when he felt pain in his lower back. On Tuesday, December 01, 2010 the physician sent the employee out for an MRI and took him off work for a low back strain.
The employee moved a rock truck and the bed free fell there fore shaking the employee. His back & neck was bothering him. He said he didn't need to go to a doctor. Went to doctor at later date. WE WAS NOTIFIED 03/14/2011 OF HIS PROBLEM AND ABOUT THE DOCTOR WANTING HIM TO BE OFF WORK FOR THERAPY. accident date 11/19/2010 - off work date 03/14/2011
Was walking returning to loader, his foot hit a round rock causing him to fall on his right side hurting his shoulder and arm.
EE was trouble shooting a ground fault in the master station on the miner. After isolating the control breaker and transformer, he started hooking heads back to the control breaker. He had locked the main breaker, but did not lock out at the power center. While connecting leads, the EE removed trouble shooting gloves, his hand came in contact with a lead on the main breaker.
While walking down stairs to basement, employee says his back gave out and he fell on stairs.
Walking to the parking area after his shift, the employee stepped in a depressed area covered by snow, jarred his back and experienced pain in his lower back. At the time, neither first aid nor medical treatment was sought.
EE stepped off of mantrip and twisted his right ankle.
Employee was adjusting excavator screws with an industrial cricket when he hit his face on the left side. He feels intense pain on the left side of his face.
Employee states that while cleaning the right side windows, he stepped onto the boom and step. His foot slipped and he caught all of his weight with his left arm. He felt something in his shoulder(pain).
Welder was using an air arc torch to remove liners from a truck. He finished up & removed his welding PPE. He was walking over to office to use radio when he saw the ground clamp for air arc and decided to remove it so he wouldn't forget to before he moved the truck. The clamp was still hot from cutting job resulting in Burns to the right hand palm.
Employee was splitting stone when a piece of the chisel broke off and became embedded in his left hand.
EE was shoveling dirt off the highwall miner beams and felt something pull in his lower back.
EE was attempting to clean out under our wash plant when he was struck by a rock from our power screen conveyor.
Employee stated that he lost his balance, slipped and fell landing on back.
Injured employee was placing a crib block under the continuous miner cats when the operator raised the miner head and smashed the injured's left hand between the cats and block. Diagnosis: Fractured left little finger and 4 sutures on the posterior side of the left hand.
The injured person was traveling outside on a 3 wheeled personal carrier and hit a sink hole, which resulted in the injured person hitting his head against the mine roof, injuring his neck and back. We were made aware that he was going to see a doctor on 12/02/2010, resulting in this filing date.
Employee was moving a wooden pallet when the pallet moved causing the employee to jerk his hand from underneath the pallet. This caused a laceration to his left index finger requiring stitches.
Employee was using a hammer & chisel to spread apart an equipment part when the chisel flew from part and hit employee, knocking off safety glasses; resulting in injury to left eye.
Employee was helping to make a belt splice. He was reaching on the belt removing the loose belt nails when the employee's co-worker accidentally hit him on the hand with a hammer. The injury resulted in a broken index finger on the employee's left hand.
Wednesday, November 17, 2010, at 7:30 p.m. the employee was driving a Ford F250 when it overturned in the flux pit. A second employee was a passenger. Both employees were taken to LaGrange Memorial Hospital for treatment / observation. This employee was diagnosed with Scapular strain (right), lumbar strain (left), and a 10 cm abrasion on the right leg.
EE deposited a pallet of product in storage area with a forklift. When backing up, he and another forklift backed into each other rear-end to rear-end. The other lift driver, not injured, said they did not watch where they were going - equal negligence. Injured EE's hat was knocked off, he felt discomfort in his neck and left work an hour later. He is now in doctor's care.
Employee was pulling himself up onto haul truck step using both his arms and felt pain in his stomach. Seen at the ER, diagnosed with a hernia which was repaired on 11/22/2010.
Employee while bolting roof fall, swung his drill head out to install outside bolt, placed his left hand on the top of the roof bolter canopy and crushed his hand between the canopy and the brow of the roof fall, breaking his middle and ring fingers.
Employee was washing floor with water hose after using it he threw hose down and nozzle hit the floor causing hose to come on full stream and hose whip lashed up and nozzle hit him over the left eye.
Employee was exiting the cargo area of his company assigned Sterling Acterra 4900 service truck after retrieving additional tooling, he slipped and twisted his right ankle.

Wednesday, November 17, 2010, at 7:30 p.m. the employee was passenger of a Ford F250 when it overturned in the flux pit. Another employee was driving. Employee lifted pump and set it on ground. Pump then turned over on ee finger causing injury.
He stepped out of his low trac onto a piece of coal, twisting left foot.
Employee was walking from #7 to #6 through the crosscut, when he went through the curtain he stepped on a piece of timber that had been sawed off and twisted his right knee. He was taken to hospital and told to see a specialist who placed him off work the following day.
At the Rock Plant, a 3"" diameter shaft approximately 24"" in length was being used to pound on the hopper wall. The operator placed the shaft up on the tail pulley guard. He turned to talk to another operator when the bar fell on his right foot.
Injured bent over to pick up a crib block. He stepped backwards and stepped on another crib block twisting his right ankle on it. He was transported to the hospital where he was diagnosed with a sprained right ankle.
Conditions were clear pickup truck engine running (diesel). EE was taking shoring blocks off of jaw crusher. Both men on ground and man on jaw made eye contact and what man on jaw thought verbal also. Man tossed off block and struck other man on the ground in the head, knocking him unconscious. Man on ground had to get stitches and staples in his head.
Got out of mantrip car, stepped up and twisted knee. Started missing work 11-16-2010.
EE filled fluids in EQ. then went to put charger on the loader, he bent over and felt a sharp pain in his back. EE is complaining of severe pain and is being taken to the local hospital.
EE was welding on a pipe, when slag from another welder hit his right jaw. The cut was small and was treated with antiseptic and a band aid. The infection he got later is due to his immune system.
While drilling to install a roof bolt a piece of roof rock 1-foot x 2-feet x 6 to 8 inches broke off striking him on the left side of his face and left forearm and right foot.
3 ee let off the tail pulley to make a lace. After making the lace they were trying to tighten the belt with the winch and noticed the brake was set on the winch. 1 ee was holding the handle of the winch with both hands while another ee released the brake and the handle slipped out of ee hand and hit him in left eye.
EE was lifting Oxy-Act bottles and pulled something in right arm and elbow.
EE's were lifting belt stand out of the man trip. EE felt pain in his lower back. EE went to the doctor and went back to work but he has had constant pain and just started to miss work on 11/21/10.
Dozer operator claimed small jar caused onset of cold sweats.
The IE stated that he was changing the batteries out on the coal hauler he was operating. When he was tramming away from the set of batteries he ran over the jumper cable with his front tire causing the cable to get tight, pinching his hand between the edge of the fender and the cable.
While changing hoist ropes on a shovel, ee right foot was sucked in between the equalizer and ropes. The tension on the rope got tighter, clamping the ee foot. Xrays were negative for fracture, diagnosed with a strain. Released to restricted duty, on crutches and boot. Placed off work.
Employee stated that he suffered a jarring injury to his neck while operating a 685 Dresser Haul truck. Medical records concerning this event were not made available until 11/23/10.
Employee was installing roof bolt when the bolt kicked out of drill chuck while rotating. As employee tried to push bolt back over the chuck, the bolt hit his right hand, resulting of bruising on the right hand.
Miner was pulling on the rope that raises the loading spout used to load trucks from over head bins when he felt a ""rip"" or a ""burn"" in his right side of his abdomen. He was diag. as having a hernia. EE is still working but may have to have surgery.
Driver was opening 4"" product valve on pheunmatic trailer to off load bulk crushed lime. Valve was stuck and he jerked on valve handle and said he felt pain in right sholder.Doctor said he had pulled muscles in right sholder and restricted him to no pulling or pushing of 20 pounds or more
Stepping off ladder way of 992 loader, felt pain. Possible pre-existing condition not related to mining.
Employee was climbing stairs to the sand and gravel plant when his foot slipped. He was holding onto the railings, so he did not fall, but reported that he twisted his right knee.
Slipped while walking beltline, when tried to catch himself felt pain in right shoulder. Had surgery on 4-15-2011, began missing work then.
Getting in truck he slipped and hit the top step injuring the right side of his chest.
While using a jackhammer to break up a concrete floor, the jackhammer fell and when he tried to hold onto it, it jerked his left arm and shoulder. Caused a strain to his left shoulder.
Employee twisted knee while getting out of rock motor. Started missing work 1/13/2011 for knee surgery.
Unit 1 was traveling W on old trace road. He was approaching a paving crew which was paving the W bound lane. A flagger waved him around into E bound lane to go around the paving crew. As he was going around, EE's left side wheels dropped off the E bound shoulder. Due to the steep grade, this caused him to overturn onto its L side, coming to rest on the E bound shoulder.
Injured was picking up lifting jack for bolter and stained groin area. Did not start missing until 12/6/10.
EMPLOYEE BENT OVER TO PICK UP THE MINER CABLE WHEN HE FELT PAIN IN HIS GROIN. EMPLOYEE DID NOT START MISSING WORK UNTIL 12/20/2010.
Employee was helping to carry a 6"" steel airline on rough terrain, when he twisted his left knee.
Injured EE had doubled back and approaching the end of his second shift. He was in the process of unhooking cutting torches on the back of the welding truck, when he fell off the truck, staining his left hip and shoulder.
Employee was installing roofbolt, the bolt hit a crack in the top and bent over and hit him in the right ear.
A pipe being lifted by a crane. Fell pinning the victims wrist underneath it. His wrist hurts now. He also has lumps in his arm. The doctors have given him pills for.
Victim was using a cutting torch. His shirt caught fire and he sustained 1st & 2nd degree burns to his hands & abdomen. Treated & released at the hospital. Still off work to this date.
Outside of quarry checking discharge water line. Stepped off a rock about 18 inches high into snow-covered area. Stepped on rock, twisted left ankle, resulting in strain and fracture.
EE and a contracot were holding a gunite hose as the cont was attempting to task train him on how to appy the gunite properly. The pressure in the gunite hose built up and eventually released, causing the injured EE to lose control of the hose. The injured EE was struck in the facial area by the gunite material, causing abrasions to the right side of his face.
Employee fell from the front access ladder on a haul truck to the ground.
Employee was welding on bucket of front end loader in garage at facility. Metal sliver from welding went under safety goggles and entered eye. On 11/22/10 employee went to urgent care facility was then sent to eye specialist who removed sliver with magnet. Employee missed one day work due to incident and suffered no damage to eye.
Employee was filling sample jars and needed to replace the jars. The box of jars was on top shelf. When employee pulled the box down, it fell, causing the weight of the box to pull on EE's shoulder. *Note: Jars are filled with sand.
The individual pulled a pallet off the Lo Trac fork lift and mashed the ring finger on his left hand. He developed staff infection and was hospitalized.
EE was operating front end loader in coal pit when the seat bottomed out causing pain between his shoulders. EE did not tell foreman he had a work related accident until he was at the doctor's office on 11/19/10.

EE was shoveling coal off of belt #2 when a large rock fell on him crushing his foot.
Injured was in 777C Truck getting loaded with 992D 101 Loader when rock came out of bucket and jarred truck and driver. Truck driver was complaining with neck and head hurting and hands going numb at 7:30 pm.
A mechanic was searching for the cause of a leak on a loader, # RI 7400. The EE went to the top of the engine compartment to pull off a cover inorder to see the area for the engine. The EE removed the bolts holding the cover in place and grabbed the handles provided to remove the cover. While pulling the cover a knot appeared in his groin. The surgeon will see him on 11/19.
Changing a tram shaft on the shearer. He was on the face side of the machine and while under a shield, the coal face rolled out and struck him on the head, neck and back from behind. The piece was 4' X 3' X 2"" thick.
Employee stated that while operating a shuttle car, he got his foot caught and twisted his ankle.
EE was checking the hydraulic oil level; he lost his balance and fell from the dozer platform.
Injured member was rolling up the wash down hose on the continuous miner when a rock fell from between the roof bolts and struck him on the right side of his body injuring his right shoulder and legs. He was diagnosed with multiple contusions and excused from work.
The employee was replacing the liners in the Mill. The employee went to replace a loose liner when the liner fell striking his right shin and foot area. Causing a fractured metatarsal bone in the right foot.
Injured was lifting 60"" top onto rail and felt a burn in his lower back. Injured employee didn't start missing work until 3/2/11.
Employee opened up chute door to look for stuck object. Door struck the back of employees head, causing his face to hit the chute.
The operator was holding onto drill steel while lining it up with the chuck. When the chuck came into contact with the steel, the steel kicked back striking the operators left hand between the thumb and index finger. Resulting in several broken bones.
EE was stepping along belt line and twisted his left ankle.
Victim was climbing up stairs on endloader when his feet slipped while attempting to grab hold to keep from falling.
Employee alleges he was working on the top deck of the primary screen. He slipped with a utility knife in his right hand and he cut himself on his left forearm. He received stitches.
Employee was dismounting from driver's side of a Lube truck. Employee's foot slipped, and fell. Employee broke fall with his left hand, which was hurt in the fall.
Injured employee was working on #81 diesel mantrip and his left hand came in contact with the flywheel while the engine was running. Employee suffered avulsion to 4th finger and fracture to 3rd finger on left hand.
Employee was working a mini bag full of additive in the hopper when his hand caught a piece of sheet metal located next to the hopper cutting his left middle index finger. The sheet metal is a wall next to the hopper.
He picked up the ends of two ventilation tubes when he felt a strain in his back. *The operator disputes the allegations set forth in Citation #8051125 and submits this report solely to terminate the citation.
Raised up in railrunner in low top and was pinned between railrunner and mine roof.
Employee's right hand was pinched between a spiral bar and a beam.
The employee was pulling a pin out of a highwall miner beam, when the pin got stuck and he attempted to pull harder and he didn't use his legs to pull the pin, he instead used his back and it resulted in a strained back.
Employee was attempting to enter a closed door to the maintenance shop with his hands full of parts. As he was opening the door he placed his foot on the edge of the cement pad in front of the door, lost his balance and strained his back.
Employee began missing 1/24/2011 for surgery on left knee. On November 8, 2010 employee twisted knee, but did not seek treatment at that time. This is the first lost time with this injury. Has not been released for work since surgery.
Member was tramming miner out of #3 entry on #1 section. He turned and was walking away from miner, when he stepped in a hole, hyper-extending his left knee. Member did not start missing work until 11/15/10.
EE stated that while working on the belts, he stepped in mud and twisted his leg.
EE was unloading belt structure from a supply car into a scoop bucket. Two pieces of top structure (rollers) slid together when he had his right hand placed between them during removal from the car. When this occurred, the injured EE's right thumb was caught between the two rollers, which caused a fracture to the tip of his right thumb.
EE was bolting on the right side of the 463 Roof Bolt Machine. He reached toward the roof bolt machine tray to grab a half header and slipped off the platform, bruising his right shoulder, left elbow and left hand. EE finished his shift and started missing work as a result of this incident on 11/12/2010.
Was repairing big sifter leaks and when I picked up the door and turned I felt an instant pain in my groin and abdomen area. It felt like a strain or pulled muscle. Was diagnosed as a hernia 12/8/10. See Attachment.
Employee was mounting a 785 hauler, when his foot slipped off the step causing the employee to fall to the ground, landing on his feet causing injury to his right ankle.
Injured employee was bent over picking up a belt rail as he raised up with the rail he began to experience lower back pain. He finished his shift but did not return to work the next day. He was seen at the ER and diagnosed with a lower back strain and excused from work for two (2) days.
Miner was fueling and greasing D8N Dozer to start stripping burden. He was bending over to grease ripper on the back of the machine, as he straightened up, he pulled a muscle in his lower back.
He was setting a jack on the corner when the rib rolled out and struck him on the head causing a concussion.
EE was taking cable down as roof bolter was backing out. The operator turned the cable reel on and the cable jerked off the hangers, striking him and driving him to the ground. He continued working his regular schedule through 1/3/11, then Doctor ordered a 20 pound maximum lift.
Steering on our 970F loader was hard to turn. EE said his arm was sore from steeing the loader, EE was put on light duty for 2 weeks.
This injury was discovered during a Part 50 Audit, we have little documentation on this incident, information was gathered from Workers Comp loss claims report. The injured hurt his lower back, received medical treatment and missed work on the following day per payroll records. When interviewed by myself and MSHA inspector, he could not remember the incident.
Miner was getting down from the continuous miner when he stepped on a rock and twisted his knee. 11-11-10 1st day missed work.
Employee was breaking clods with a sledge hammer when he injured his hand. He also suffered a lime burn to his wrist causing cellulitis
Employee reported while reaching over to close the door on the loader, he felt pain in his lower back. Pain developed into severe muscle spasm requiring medical evaluation. He was restricted from full work duties as a result of this injury.
EE was assisting in supplying the miner in 5B. While carrying roof straps EE felt a sharp pain in his left shoulder after lowering arm. EE cont. working and did not miss work until 2/21/11. EE went off work for shoulder surgery on 2/21/11.
Employee was lying under a Kubota tractor on his back removing a drain plug. The plug was tight, so the employee decided to brace himself with his left leg against the tire to get more leverage. As he turned the wrench he felt a crunch in his lower back.
Employee performing maintenance on top of screen deck. Employee lost footing and fell on deck fracturing wrist.
Employee was jacking loader to change tire when loader fell. Employee stated that blocking material broke causing loader to fall. Employee sprained rt foot and broke left side clavicle.
While tightening a bolt with two hand wrenches, he felt a sharp pain in his ribcage. Upon X-raying his chest, it was discovered that he had fractured a rib due to overexertion while attempting to tighten the bolts.
Employee was shoveling in the aggregate plant as the ground worker on 11/8/10. He called and said his back hurt.
Employee was walking across the feeder and slipped and twisted his ankle, causing a sprain.
Injured employee was accessing the operator's cab on a D9R dozer. As the employee opened the cab door, he stepped backwards missed step and fell onto the dozer's track below fracturing a vertebrae in his neck.
Employee was attempting to climb on PL63115 Stacker stepped on a chunk of coal, lost her footing and twisted her right knee. First day of missed work was on January 31, 2011, due to surgery.
Employee tripped on debris and fell forward attempting to catch himself he struck his left forearm on the control panel of the MSA rock dust tanks. This resulted in a fracture to his ulna bone.
Employee stood on the step of the loader to see if a wire was off the starter and when he stepped down his left calf was cut by the step.

Employee went to Dr. on 11/06/2010. Xray confirmed two fractured metatarsals in right foot. Employee was wearing metatarsal boots at time of injury.
While shoveling the slope belt, employee felt pain in his right knee. Employee worked all scheduled shifts until going off work on Jan 7 2011.
EE was removing a jammed piece of concrete from the collector belt of the gravel in-feed bin. The concrete fell and crushed his left index finger against the angle iron frame of the bin causing a laceration.
EE was going up to the face in the #6 heading, when he stepped on the #2 continuous miner cable and twisted his right knee. Seam height was approximately 50"". He was taken to ER and was released with no restrictions but continued to experience pain. On Thursday December 16, 2010 he had surgery to repair a torn meniscus.
EE was bending over machine all day and his back was sore the following day. Did not report to HR till 11/14/10
Accumulated mouse droppings in an electric cabinet caused an explosion when the operator was shutting down the compressor. Burns to his right hand.
Employee was pulling a track pad off of a dozer. His hand was caught between the master pad and weights. His right index finger was broken.
Employee states he experienced back pain when lifting stone on Nov. 4th, thought he was OK, but then on Nov. 5th he had more pain when wrapping a pallet. Rested during the weekend and said he felt good in the Morn. on Nov. 8th, after working for a while the pain came back, he reported to manager. Employee went to provider Nov. 9th AM. Doctor told employee he has a back strain.
Employee was driving a stock truck on quarry roads when he hit a ridge in the road causing him to jar his neck.
Employee tripped on metal platform stair on 5th floor of autoclave. Was taken to the ER, x-rays positive for left tibia plateau fracture requiring surgical repair.
EE was lifting lid to powder box when he felt pull in lower right side back. Awaiting personal/occupational determination by doctor- received confimation from specialist 11/30/2010 that injury is attributed to work making this a lost time accident.
Belt man was splicing stacker belt, had belt apart and was getting ready to spice belt kicked on and caught leg in belt structure.
Employee was riding a rubber tire man trip out of the mine when the man trip hit a rib hurting the employee's lower left back. Employee did not start missing time until 11/09/2010.
Employee was removing rusty bolts from a discharge flange, using a 1 7/8 inch wrench.

Injured was cleaning up coal he had spilled. He was shoveling it back into the bed of his trailer. When letting his tailgate back down he had propped open he had a shovel in his left hand and the tailgate in the right hand straining a tendon in his right arm.
EE jammed finger into heater/fan to see if it was working, it was and fan chopped off the tip of EE's finger.
A piece of coal struck employee in ankle. Medical was sought, no fracture, told ankle would get better, full release. Ankle not getting better, appointment was scheduled with orthopedic DR., before appointment there was minor re-aggravation. Feb 10, 2011 he attended scheduled appointment, became LTA with soft tissue injury. Late reporting due to medical records gathering.
Employee was working in open conveyor area at the time. Was welding 2 brackets on airline but was not wearing respirator under welders mask. Injury is still undetermined. After investigating work area, the injury is still undetermined, and if other safety measures are required we will investigate further and take proper steps to correct.
Employee was boarding a D-10 tractor. As he grabbed and put weight on the handhold on the hard nose, the handhold bent downward,causing employee to go forward and down, causing his foot that was still on the bladearm step to be bent in an awkward position.

While dismounting truck, the operator slipped on top step loosing footing and fell to the ground.
Employee was bolting top when a rock fell striking his canopy which caused him to run. While running, he tripped and fell striking his head and the RT side of his body. He was knocked out for 30 seconds. He was complaining of pain to his RT side, chest and left knee. Injuries were due to fall and not from rock falling. Was transported to Harlan A.R.H.
Climbing out the kiln room window to gain entrance to catwalk to work on walls.
While fueling Cat 992C Loader, the operator had his left foot on the bottom rung of the access ladder and pulled the fuel hose towards him and the hose caught on a rock causing the employee to lose his balance and fall backwards hurting his left ankle and hitting the back of his head several inches above the neck.
2 employees were to weld a broken hand rail & replace a v-belt guard. The 2 items were 5 feet apart. They decided to weld the handrail & then replace the guard. For reasons unknown the injured employee started replacing the guard while the welder was repairing the hand rail. This resulted in a flash burn to the employees right eye. The employee did not work his next shift.
While another employee was lowering a screen down it slipped and hit employee on left foot causing a contusion.
EE was making a hole in a block stopping upper right corner with a tiack hammer, when attempting to strike the wall the hammer hit the metal strap and dislodged a piece of rock 12' dia, ee was struck on the right shoulder. EE complained next day loss of grip and some numbness to the right side of neck.
Employee had gotten out of drill and picked up an orange cone to mark his drill hole rows. When he turned he stepped on a rock and twisted his right knee.
While hanging feeder power cable from roof, employee heard a ""pop"" and felt pain in his right shoulder. The employee subsequently had surgery on February 3, 2011, which was deemed ""worked-related"" by the attending physician. Because the employee was able to work for three months, this case was disputed. The employee returned to work on full duty status on April 18, 2011.
EE was busting rock out of walkway on 2 Belt bk-18 with a sledge hammer and his shoulder started hurting. He was taken to hospital and given off until 11-4-10. He returned back to work on 11-5-10. He was diagnoised with a strained muscle.
He was trying to get the top piece of drill steel out of the hole that he had just drilled. He had his left hand on the drill pot, while working on getting the steel out of the hole, when he pulled the bottom piece out of the hole the top piece of steel came out and hit the left index finger.
Employee was operating loader. Employee backed over auger hole and hurt his back.
Employee was working on fan trying to remove a coupling. The center bolt would not come loose. Tried using a large break over bar, then used a large pipe to get more leverage. When applying pressure the adapter broke allowing the employee to go forward twisting lower back as he went down. Employee is on restricted duty.
The 7MN recovery crew led by a Twentymile employee was heading in by on 3mn entry at the bottom of fault (starting to climb). As the mantrip started to climb the fault, the operator immediately noticed a scoop pulling a trailer which was hidden by the grade/top. Operator attempted to brake but was not able to stop. The mantrip crashed into the trailer being pulled by the scoop.
Employee was helping shovel rock off of a conveyor when he felt a pop in his right shoulder.

Employee was attempting to open a door on a railroad box car. The employee was pulling down on the handle and felt a sharp pain in his hip. The pain radiated down to his leg.
Employee was attempting to free a 12 foot section of air line from its hangar when he slipped on mud coating the sill between the rail and rib. He fell to the sill on his backside. The accident did not involve any mining equipment or protective devices. The work being performed was not part of a mining method, but a safety upgrade.
Was at 3c Head taking roller out with chain ratchet. Belt head came off frame hitting right shin, roller fell and hit knee on right leg.
Splitting rock slab, slab broke and caught index finger as it slipped to the ground.
Injured was kicking with his foot a 8""x8""x3' crib block under a fifth wheel trailer so he could un hook it from a prime mover truck. He missed the crib block with one of his kicks and felt a sharp pain in his left knee falling to the ground and laying there until he received help to get up. Employee started missing work on December 14th 2010
EE stated that he was roof bolting when a piece of draw rock fell striking his right forearm. EE was treated at the ER at Middlesboro ARH where xrays showed no fractures. EE went to the Dr. on 11-3-10 and was admitted to the hospital with cellulitis in the area of the injury.
Employee was climbing up ladder into loader and his knee gave out.
Lifting steel c-channel plate, strained muscle in left upper arm.
Two bolter operators were re-bolting an area that was breaking up. They had bolted to a simplex jack. They decided to manually remove the jack. While one bolter started to let the jack down the other bolter pulled back on the mesh. At this time a rock fell out of the roof between the roof bolts striking the bolter pulling the mesh back on the legs causing a fracture.
Employee was helping a mechanic pick up a metal plate from a D11 dozer when a sharp pain hit in the middle of his back causing him to fall to his knees.
Mine operator sitting in seat of excavator turned to see what other miner wanted as he had yelled. When he turned in the seat coat sleeve caught swing lever and swung excavator into other operator's foot.
Around 6:25 the driver side rear wheel of the gator became dislodged causing the vehicle to turn sharply to the left. The gator struck the rib and ejected the employee, with his right thigh striking the vehicle and fracturing his femur.
Employee walked and crawled to back of roof bolter, when he turned to go around the bolter he felt pain in right knee. Employee has worked full duty since 11/30/2010. At 01/18/11 doctor's appointment he was instructed to work modified duty with surgery to be on 01/24/11.
Employee slipped on wet bridge board in the walkway. When his feet went out from under him, his arm got hung over the top rail of the belt structure. Employee's body hit the ground but his arm stayed caught on the belt rail. Lost time reportable injury 3/10/11 due to surgery.
Employee was installing roof bolt when it bent, he put his hand on the bolt and it twisted his right hand/wrist.
Employee was preparing to go through a mandoor when he slipped and fell in the mud twisting his left knee. Lost time injury as of 3/7/11 due to surgery.
Employee was removing frozen ore from semi bed, chunk of ore released from bed and struck employee on left foot pinning foot and causing employee to fall backwards resulting in fracture to left ankle.
Mine Mechanic exited the service truck and felt a pain down his back and left leg.
Employee climbing over railcar, missed a step and tried to catch himself. Felt pops / snaps in elbow.
Employee had turned around backwards to look at the bottom rollers when his left leg slipped out from underneath him and twisted his right knee. Lost time reportable injury 02/14/11 due to surgery.
While operating a Continuous Mining Machine on MMU 001-0 section in #8 right x-cut (punch thru), EE was struck on top of his shoulders with a piece of mine roof that appeared to be secured to the mine roof, became loose while mining #8 x-cut into #9 entry. The piece of rock fell from between permanent roof support and the coal rib.
EE was shoveling loose coal on belt line. He felt a tightness in his lower back. He has been on light duty until 1/4/11. He is currently rec. treatment and will return to work at doctor's release.
Lost footing in loose rock and dirt, injured ankle.
Employee was driving a haul unit on the main haul road and collided with another haul unit that was turning onto the haul road from a side road. Employee was taken to ER via EMS. Released from ER same day with minor injuries. Haul unit sustained disabling damage.
Employee was walking by fan carrying some grating to make a walkway in the tower area. It was muddy and the way he had to position the metal to get through the walkway he slipped and hit the edge of the guard with his right forearm.
Employee was removing a filter from a 993-K Cat loader. As he put pressure on the wrench, he developed a pain in his right shoulder.
EE was moving a tire on a SkyTrac Forklift, the tire started to tip over. He caught it and he said his felt a sharp pain in his shoulder. He reported it to his supervisor and an incident report was completed. He began to develop extreme pain on Sat. Dec 3 and went to the ER where he was given shot for the pain. He was taken to the company Doctor on Dec 6 where he was evaluated.
Employee was greasing the drive gears on the waster screw when his sleeve came into contact with the gears causing lacerations to his left arm.
Walking and tripped, fell backward and strained back trying to break fall. Resulting in acute lumbar strain.
While using an air lance to remove kiln feed build up in preheater tower, employee tripped over support beam. (Did not become reportable until he started to miss work due to surgery on 2/24/11.)
Employee was bolting. He repositioned the jackleg to finish driving the split set when he felt his back ""pop"". Pain was in the lower back.
EE was tightening bolt, the bolt broke and the EE dropped the wrench on his hand.
Employee was getting out of a stockpile loader and hit his knee on the door frame. We filled this out online on 11/18/2010 and reported on our 4th Quarter report. It wasn't until today that we realized that we had not pushed ""Submit This Form to MSHA"" after completing online. Sorry for delay.
Employee was pulling shield 120 from shield 119. Shields 119 and 120 had a loose relay bar so when employee pulled shield 120, it pinned it into the pan, also pining employees left ankle between the relay bar and the pan. Lost time injury.
As the individual was traveling underground on a Mac 8, his dinner started to move. He reached back to catch it as he started under an overcast, catching his left ear causing a laceration which required sutures.
Was shoveling along reclaim belt and was attempting to put material onto belt when he felt slight pull in stomach/groin area. After completing shift noticed a slight bulge in belly button.
Was helping put augers into beams and twisted the wrong way causing ee lower back to knot up. Didnt really feel anything until Friday morning around 10:00 A.M.
While walking through the warehouse and employee was struck by a forklift which caused his right ankle to be broken.
EE was using a torch to cut bracket to change water hose,accidently cut hydraulic hose and threw oil in his face and hands (burns).
Doing maintenance on crusher cleaned lime stone from pulley ""after completing maintenance"" the crusher tail pulley was moving got screw driver caught in pulley. Pulled right arm through pulley.
Employee was walking on the Raw Mill Deck with a grease gun in left hand. He started to fall and grabbed the handle of an empty tool cart with his right hand. It tipped over letting him fall on his right knee.
Employee strained back while performing clean up activities.
Employee was using a chipping bar to clear chunks from the Feeder.As employee was poking a chunk he was flipped by the bar as the chunk fell onto the pry bar. Employee fell and struck upper right side and thumb on the mid-rail of the floor handrail system.
EE was operating an Joy CM in the #7 entry when a rib rolled off hitting him on his right ankle.
EE was squatted down with a slate bar in his hand and felt a sharp pain in his back. The pain became more intense. EE was transported to Greenbrier Med. Center. This report is to abate citation 8154309.
Employee stated that he was walking down the #2 entry,when he fell onto his back. He was transported to the hospital with complaints of numbness in his left arm. He was examined with no fractures or abnormality. He returned to work on light duty 11-10-10 and was on call for Dr. apt. to give full release. He missed work on 11-09-10 while at hospital.
EE was checking tanks on 1323 grease truck. When he was exiting rear door on bed he slipped to the ground, first on right foot and when his foot made contact with the ground his heel was up against the bulk antifreeze meter. When the EE brought his left foot down to the ground his balance was off and he fell and hit the bulk pump with his lower back. EE has lower back pain.
As the miner operator pulled the continuous miner back after sumping in the roof channel in the channel rack caught in the right rib. The channel flexed and sprung out of the rack and struck the injured in the back of the head and neck on his right side. The injured was standing at the rear left side of the full face continuous miner.
Employee was placing a bottom belt roller in a hanger when the roller fell striking him on top of the foot. The employee worked until 11/29/2010 at which time he was unable to continue work.
Employee stated that he was mounting the left rear inside tire onto a mechanic's truck. While mounting the tire, the employee's left ring finger got caught between the tire rim and the hub severing the tip of his left ring finger.
Employee was positioning screen cloth on a JCI 6X20 screen and twisted / strained his back.
The employee was reaching across the top of the continuous miner to pick up the screen to wash it out. While doing so, a rock fell out between roof bolts striking the top of his right hand. The result of the injury was a possible fracture and four (4) stitches to the palm of his hand.
Mechanic replacing a blown hydraulic hose on a 365 Cat Excavator. Mechanic bled pressure from the pilot control, lossened the hydraulic tank plug, and relieved pressure from the tank prior to maintenance. However, pressure built back up in the line and during the removal of the hose the pressure released, sprayed and contacted the mechanic's eye.
Employee was handling a filter pot header pipe attempting to place the first bolt in the flange when the pipe slipped and caught one of his fingertips on his right hand against the filter pot. He was wearing leather work gloves.
On the date above a Volvo BM L150 was dressing and pushing off a dump. A CAT 777 Haul truck was backing to the dump to drop a load. The loader operator didn't see the truck and backed into his path. The loader collided with the rear tire of the truck. The loader operator reported he jarred his back during the accident on 11/12/2010.
Employee was running a Jackleg. Employee pulled back on the machine and right foot slipped causing employee's right knee to twist.
Shaft used to secure belting slipped and struck employee's head. Employee fell as a result of the blow to his head and struck the back of his head on the floor. Employee suffered a laceration to his head and possible concussion.
EE was pulling ore chute, motorman was signaled by EE to move ore cars into ore chutes. As ore cars came in EE lost his footing and ""R"" foot slipped in front of 5th wheel on ore car.
EE was attempting to pull a welding trailer approximately 9 feet to the pickup truck hitch, in anticipation of moving to another worksite. Uneven ground caused trailer to roll out of control, which caused EE's hand to become caught between receiver hitch and the tailgate, resulting in laceration to the left hand.
Employee was tramming the manlift from his work area to the parking area. While driving the left front tire went into a pot hole causing him to become off balance. His foot came off the control causing the machine to make an abrupt stop. Employee fell forward striking his upper right leg on the basket inside the manlift cage.
While shovelling by a belt the employee stated he hit his left pinky finger on the handrail to the stairs and fractured his little finger. The employee was sent to a surgeon and the finger was pinned. He is currently on light duty.
While driving out liner bolts the bit of the jackhammer became wedged in the hole opening causing the hammer to pull in towards the shell of the mill. This action caused the employee's hand to become pinched between a mill rail and the trigger of the hammer. This caused severe pinching of all four fingers on left hand.
Injured worker was pushing a water hose in a mis-fired hole when he cut his finger on a rock.
Using a pry bar to clear rocks and wood from under a leaking 14"" water line. Pushing back and forth on pry bar when it shifted sending employee forward into the catwalk sliding his little finger down the edge slicing his little finger open.
Lifting pump from machine to grab overhead hoist, turned to set down and stumbled. Pump pinched finger against frame of dozer and cut finger.
EE's were cleaning the raw shale buildup from C-34 tripper chute. While cleaning the chute, IE was using a pneumatic chipping gun and when IE tried to change his footing he slipped and hit his left calf muscle on the inside of the chute.
EE was throwing 100 pound bags. Felt pain in wrist at the end of shift. Went to doctor on 11/30 and was told EE had tendonitis. Restrictions of not lifting anything over 10 pounds was put on EE resulting in MSHA reportablity.
Employee was placing his tool box into Bobcat when he felt pain in his back.
EE was walking on snow packed ice from coag distribution site. Stepped over white pipe then started over black pipe when his left leg slipped out from underneath him. Injury - right knee acl tear.
EE had bent a roofbolt and prepared to drop boom when he accidently back spun the wrench striking his right wrist with the bolt.
Operator car stalled, emp. got out of the car to check breaker. When returning to car emp. stepped on a rock and lost his balance, his left knee shifted and popped. Emp. did not fall. Later when trying to stand up in ram car to get out, left knee buckled again.
Lifting tool boxes in tractor and felt pain in abdomen. Did not become reportable until surgery on 11/29/10 to repair hernia.
Employee's knee popped while walking up incline at plant. Management saw employee limping a week later and questioned if he was hurt on the job. Employee said no. On 12/8/10 management received call from doctor saying employee was there saying he had work related injury. Employee was put on restricted duty until release of 12/30/10. No tear in knee.
Walking up steps from maintenance shop by hardinge mill area fell backwards. Held onto guardrail and pulled right shoulder during the fall.
3700-185DC - went to heading 37-185 to watch jumbo drill, walked back to shop and tripped, tried to catch himself and fell on a rock bruising right side lower rib area on side and front.
The employee was constructing a brattice between the #2 and #3 entries. While working in a restricted area between the brattice and the rib a section of the rib measuring 63 inches long, 21 inches wide and 4 inches to 11 inches thick fell striking him on his lower back and left leg.
He was walking and slipped on some ice that was in a shadow and under some snow. He broke his ankle as a result of the slip.
Employee was taking pin out of clevis that was located on the face plate. It had a bar in the hole to loosen it and the bar slipped out of the hole and mashed his finger on the shell.
Emp. was unloading air boat off trailer into the water. To get the boat to start turning away from the bank emp. had to push the boat using the back of her legs to get the boat to start turning. Emp. felt the back of her right knee strain.Emp.taken to Work Comp. doctor and restrictions given on 11/23/10.
EE states he noticed what felt like a blister forming around 14:00 hrs. At 15;30 hrs while changing out of his rubber boots he noticed some blood on his right sock. After removing the sock he noticed a burn and reported it to the front guard gate. EE was seen at medical and transported to the hospital for further treatment.
The employee was installing a hoist cylinder to the dump body of a 960E haul truck. He loosened the bolt on the shipping strap and the bolt threads failed causing the hoist cylinder to fall pinning his left shoulder between the hoist cylinder and the tire which resulted in shoulder trauma.
Employee was changing out 10"" metal pipe 9' long, when pipe fell, landing on left foot behind steel toe on boot. Employee has been placed on light duty due to this injury.
Employee was walking in the mill bldg, 2nd floor, when he lost his balance and stumbled to the floor on the same level hitting his left knee against equipment causing an open wound.
EE was taking off spare washer, and while taking off the hose and fitting on the caustic shell side washout, he had his hand on the tube directly underneath the fitting and when it came loose, it fell and struck his left middle finger.

Emp. walking to the belt controller & tripped on a turnbuckle & anchor. He fell face down on the floor. Emp. denied pain at the time of injury. He contin. to work & his left knee started hurting. Emp. was off on personal leave & notified the Safety Dept. on 12/8/10 that he was still having swelling & pain to his left knee. Emp. taken to doctor & placed on restrictions on 12/9/10.

Employee was driving a haul unit on a return route road that intersects with the main haul road. When he got to the intersection with the main haul road, he turned his unit left and collided with another approaching haul unit. Treated at ER. Released same day with minor injuries. Haul unit received major cosmetic damage.
Climbing up a scaffold ladder, our employee IE's hand slipped out of his glove. While compensating, foot slipped, He fell backwards and down 5.5 feet hitting another EE who caught him and broke his fall. IE hit his hip on the floor.
Employee was walking down a catwalk, he twisted his ankle and felt a pull on the backside of his right knee resulting in a sprain knee.
Employee hit middle finger on right hand while trying to get pie pans apart and cut finger. This accident occurred on 11-18-10 but did not become restricted duty until 12/1/10.
Employee has a chronic back problem where he periodically experiences lower back pain. On 11-18-10, the employee felt pain in his back while operating a shovel. No unusual conditions were found with the shovel. The employee did not trip, slip or fall. A letter substantiating an occupational illness will be submitted in a separate correspondence.
While taking down a welded wire bulk head miner leaned ladder on bulk head and gut the chain holding it up. The bulk head fell and the miner received cuts and punctures to the left arm.
EE was walking from the quarry break room to the quarry operator shack. The EE slipped and fell hurting his left knee and left wrist. On 12/29/2010 the miner was put on restricted duty.
Employee was opening the elevator door at the top floor of the blend silo. The door was difficult to open because it was cold out. Employee stated he strained his shoulder and back and it was sore for several days. On 3/23/2011 the employee reported that his shoulder was hurting and he wanted to see a doctor. He saw a doctor who gave him a cortisone shot.
Employee and Co-workers were building cribs. The employees were passing the crib blocks to each other. The employee dropped a crib block and reached down to pick up the crib, when his co-worker handed him another crib. The employee's right hand was mashed between the crib blocks.
The employee was operating a roof bolter. He placed his hand on top of the operators canopy and swung the boom pinching his hand between the mine roof and the canopy.
Employee was using a hammer/hatchet to remove nodule from a cathode, when the hammer kicked out, causing pain to the employees wrist. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 11-22-2010.
The Chip Plant Operator suffered a strain to his right shoulder while descending the stairs on the thickener tank. He has now received medical treatment (injection 12/13 and begin physical therapy 12/15)for his injury.
Employee was operating mantrip, when mantrip spun out on hill. Mantrip started rolling backwards, mantrip hit 20 ton diesel motor that was on track. Employee was placed on light duty with bruised left hip.
Subject stepped out of the mantrip and rolled his right ankle.
Employee was walking toward pipe valves, slipped and fell in mud, striking right shin just below the knee on bolted metal pipe flange. Employee was seen in ER, diagnosed with large hematoma below knee, given 2 days of restricted duty, pain medicine and anti-imflammatories.
EE smashed right thumb on existing flange of a pipe, while he was guiding the replacement pipe into place.
EE was using jackhammer, lost balance and fell forward catching his foot in the floor grating. Physician prescribed protective boot and limited duty.

Miner tripped on top step of stairs leading into generator trailer, he put out his hands to catch himself and broke his left wrist when his hand contacted the floor.
Was standing on a rock 15"" in diameter 8"" high, lifting roof bolter cable up to a j-hook when he lost his balance and fell to the ground.

Employee was climbing down ladder as he was pulling trolley cable and strained left shoulder. Medical evaluation resulted in left arm being restrained and is placed on restricted duty.
Employee fell from Pullman-Standard Hopper car. It is assumed employee fell from car while attempting to close hatch door.
Employee was climbing down a ladder and slipped on a rung.
The employee was loading product into a bulk truck. In the process of lowering the product chute into the opening at the top of the truck, he pinched his left ring finger between the handle of the chute and the platform railing. It was originally diagnosed as a contusion. An x-ray later revealed a tiny fracture.
Repetitive fabricating of equipment guards.
As an underground long hole driller was working in his area he stepped over a rock and lost his footing which caused him to move in an unnatural way to regain his balance. The miner was evaluated for a sprained left ankle and placed on 48 hrs of restricted duty.
Employee was swapping a tank and had difficulty operating a valve and strained himself. He felt a strain in his groin area.
EE was changing a bottom belt roller, one side in the cradle and while attempting to raise the roller out of the bracket it fell out bending the right wrist backwards. Pain to his right wrist.
The employee was washing down the deck of the dredge with a water hose. As he through the hose down the valve on the nozzle hit the deck which caused the valve to open and the hose to whip around and the nozzle struck the employee's foot and caused a contusion.
Injury occurred while changing bags on HiVac trailer. Subject and one other employee were pulling out filter cages from the HiVac trailer, when subject pulled a cage out and felt a pain in his back. Received medical treatment on 11/17 - Rx Drugs and Restricted Duty.
EE was lifting the welding trailer off the hitch of a pickup truck and felt soreness in his lower back.
Employee was using a pair of channel locks to help other employees pull on a piece of 60"" belt. The employee stated he felt a sharp pain in his right forearm. The employee did not report this to management until the following day. He has not missed any work and his first day of restricted duty was 12/7/2010.
While EE1 was holding the bottom of one side of the head pulley drive guard, EE2 lifted the other side, causing the guard to smash EE1's left 4th finger. Laceration to finger
The individual was traveling outside on a Lil Mac when he ran into an equipment door. He did not stop at the door switch. The ride was checked and was found to be in proper operating condition.
The employee was bolting. As he attempted to put a bolt in the drilled hole, using the hyd. of the bolter, the bolt bent and hit his hand which caused a strain.
Employee was moving an oil drain barrel from under dozer ripper. Drained transmission.
Individual went through a man door and when he stood up he felt pain in his left shoulder he was placed on light duty for 5 days
Employee burned hand with an open torch flame while cutting/grating metal pieces inside the finish mill. He explains that while cutting metal pieces the hose got caught on something and as he pulled to free it, the flame went under glove and burned the top part of his hand. Restrictions as follows: keep wound clean and dry with tolerated use of (L)hand.
Employee was attempting to clear boulders from a clogged chute and suffered a back strain.
Employees left foot slipped from walkway platform to step causing shin to contact step. Conditions: flat walkway with one step up to another level.
Associate descended ladder on the front of the Cat water truck. Upon reaching the bottom and dismounting he placed his feet on a wash out hose. Since he was on uneven footing he rolled his ankle causing a sprain.
Employee was helping load stone treads onto a customers truck. While carrying the piece to truck, he felt his right knee pop.
Cleaning up coal spill by coal trolley using a 6 inch vacumn hose, shovel and hands. Employee was bent over the vacumn hose and felt a pain in their lower back.
Employee was pushing the rubber basket in the pre-cleaner housing on a D11R when his left hand slipped and he struck his left wrist on the adjacent bolt.
Injured member was installing roof bolts when a piece of draw rock fell and struck his right arm just above the elbow. He was transported to the ER and diagonsed with tissue bruising. The ER physician placed him on restricted work activity.
The man was pulling a shuttle car cable when he slipped and fell to the bottom.
While getting on the crew bus employee tripped and fell. His right hand contacted the bottom step of the bus. This contact resulted in a fractured metacarpal bone behind his pinky finger. He did not seek medical attention until 11/16/2010.
On 11/08/2010 injured was placing a spool of cable on top of a undercast, when he strained his right shoulder. Injured continued working and the strain seemed to get worse. Injured went to a orthopedic surgeon on 12/10/2010 and had a surgical procedure to repair a tendon, making this a reportable incident.
Employee was struck in the right leg by a cone crusher while trying to remove the cone from the frame. His leg was bruised and he returned to work for 9 days. His leg then became infected 10/19/2010 and had to be drained and antibiotics were administered making this a reportable case.
Employee was installing a new impeller on a pump. When he was pulling on the belts he caught his fingers in the belt and the large sheave. His middle right finger was fractured. Returned to work the next day on light duty.
Miner was lifting a metal lid on a dynominer machine when he lost control of the lid and it dropped on his finger smashing it between the lid and the machine frame. The injury was diag. and treated with notification on 11/16/10 at which time it became a medical reportable injury.
EE changed the fuel filter on the dozer. He practiced proper dismounting procedures by using 3 points of contact. However, as he took his final step to the ground, his right foot contacted a piece of shale. (Approximate size: 4""x6""x4"" thick). This caused his right foot to roll laterally. X-rays determined he had a very minute chip in the bone on the right foot.
While crawling out from a screen deck, employee dropped one foot to scaffold board. The board broke and employee fell, injuring right middle finger, and slid down chute to the conveyor belt.
Employee injured his shoulder while placing pallet toppers on a pallet.
Employee was adjusting the seat of the production truck and the lever was stuck on the seat belt. The lever slipped and caught the third finger of the right hand between the seat belt bracket and the seat. This resulted in a fracture of the third finger.
