During regular maintenance on the 16' production skip, guide bushings were replaced on the south skip. While cleaning the guide ropes, debris jammed in the guide bushing causing the skip to be stuck at the 800' level. Two employees were cleaning the ropes and as a result were stuck at the 800' level

A roof fall has occurred at Old Holley Bush Submains on 001 section. The fall was located between brk. 32 and 33, the size of the fall was approx. 6 thick, 60' long and 20' wide. the area has been crib and dangered off.
A non injury roof fall occurred in the No.7 entry, this is a continuation of a non injury roof fall reported for 5/20/2011. MSHA roof control specialist investigated the non injury roof fall.
A roof fall occurred on the section while the section was pillaring. The fall was located at spad 407 in the #5 entry. The fall was approximately 8'high X 20'wide and 36' long. The fall caught the continuous mining machine and a MRS. A retrieval plan was submitted and posted and the equipment retrieved.
A fall was discovered in the 5 Mains Section (290' inby the feeder, #47 Block, 3x2 Crosscut). The fall was bolted with 6' Super Twist Bolts. No travelway, ventilation, equipment, or people were affected by the fall.
A fall was discovered in the 11 seam, 1ST. N.W. Main, #7 Entry, xc-10 (spad 7+00) Approx width = 20' Approx length = 40' Approx height = 10' to 12'
The elevator in the #6 shaft was inoperable due to a control fuse that failed. The elevator became inoperable at 8:15 p.m. on 5/30/2011.
On 05/30/2011 at approximately 10:00 p.m. there was a roof fall found in the No.4 entry. The fall was found by EE while fire bossing. This fall is located at survey station No.1708 and measured 25'x25'x5'. The fall will be cleaned up in accordance with the clean up plan.
An unintentional roof fall occurred at the New Future main South Entry #1, Entry #2 at crosscut 6. The fall was of unknown length approximately 18 feet wide and approximately 15 feet thick. The fall did not affect ventilation or passage of workers. The fall will be cleaned up.
A fire lasting more than 30 minutes occurred in a lateral flexible rubber pipe when a power failure occurred and the coal mill exhauster fan could not be restarted.
An unplanned roof fall occurred along the #3 belt at break #8
Experienced an unplanned ignition of coal in the #2 KFS Ball Mill system. No injuries or damages occurred.
An unintentional roof fall occurred at the New Future main West Entry #3, 2600' and in the crosscut between entry #3 and entry #4 at 2620W. The fall was of unknown length, approximately 17 1/2 feet wide and approximately 12 feet thick. The fall did not affect ventilation or passage of workers. The fall will be cleaned up. Fall #28B
A Rock Fall was discovered at 71 break on the track entry the fall was 20' wide x 30' long x 6' to 7' thick the area had been supported with 4' fully grouted resin bolts, no injuries, travel was blocked. The fall will be cleaned up.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE FIRST RIGHT OFF OLD #8 BOLT SPAD NO. 3144. THE APPROXIMATE AREA OF THE SAID FALL WAS A LENGTH OF 50 FEET, 18 TO 20 FEET WIDE, AND 6.5 TO 8.5 FEET THICK. THIS FALL DID NOT EFFECT THE OPERATION OF THE MINE. THIS AREA HAS BEEN CRIBBED AND DANGERED OFF PREVIOUSLY BEFORE THIS FALL. THE DISTANCE OF THE FALL TO THE FACE WAS 3,440 FEET.
A roof fall was discovered in crosscut 19, entries 2-3 North Mains. Primary support was 5' fully-grouted roof bolts. The fall was approximately 6' high and 34'long.
Weekly examiner found the roof fall located on 6 zone 43 block during his examination of the area. No air way blockage occurred and no injuries occurred fall will not be cleaned up.
Discovered roof fall in an inactive workings in return. The fall came across #2,#3,#4 entries of 2nd left B-1 Butt and continued up #9 room approx. 100' #10 room approx. 500' #11 room approx. 300' #12 room approx. 100'. Fall did not impair ventilation or impede passage.
At 11:05 EE hoist man for Irvin Construction reported the elevator in shaft 1 was not functioning properly. We notified the underground crew and instructed them to start traveling outside by our secondary escape way(Belt slope). The adjustments could not be made within 30 minutes and the hot line for MSHA and State were called by 11:40 AM
An unplanned roof f all has occurred at brk 33 of 4 belt in the #7 entry. The fall approx. 70' L. X 17' H. X 19' W. There were no injuries involved.
While mine fire boss examiner was traveling intake with MSHA inspector for the weekly exam a continuation of an existing roof fall was discovered. The fall fell thru two thirds of adjacent intersection. Ventilation and travel was not impeded. Area was dangered off. Spad #19284.
A fire was discovered in a day box on a powder truck as a shot bench was being loaded. A brush/tree fire was located near the loading site which may have been the ignition source. No injuries occurred in this incident.
A ROOF FAll was discovered in a remote drift opening in the #2 entry 20 feet outby survey station 671 approximately 25 feet from the surface. The roof fall measured 20 feet long X 18 feet wide.
Roof fall occurred in the #3 enrty at spad #1576. Fall occurred 5600 feet from the active face on the 001-0 MMU. This area was developed March 2008. Area was originally supported with 5 torque tension bolts. Roof fall did affect ventilation and passage of persons. Fall will be rehabilitated. Fall 20w X 20l X 8H. Federal and state agency were notified immediately.
While mine fire boss examiner was traveling the weekly intake travelway with MSHA inspector a roof fall was discovered in #6 entry at 70 breakacross 5 north belt. Fall measured approximately 4.5'high, 20'wide and 40'long. Area is under 103 K order and was dangered off. Ventilation and travel is not impeded.
At approximately 3:30am the man/material elevator became inoperable. After inspection it was determined that a power fuse had blown. The use was replaced and elevator returned to normal operation.
At 1:14am on 5/26/2011 the elevator located at the Hart Street Portal failed to operate correctly. The incident was called into the MSHA ""One Call"" center at 1:33 am and MSHA was notified of a major incident with the elevator and the location of the cage was unknown. MSHA contacted the mine at 6:30 am
While mine fireboss examiner was traveling the weekly intake travel way with MSHA inspector a roof fall was discovered in #1 enty at 66 break. Fall measured approx. 4' high, 20' wide and 35' long. Area is under 103 K order and was dangered off. Ventilation and travel is not impeded.
Roof fall occurred in the number 4 entry, while pillaring the left block on the 002-0 MMU. The fall was approx 22' high at its highest point & 60' long & 20' wide. Two mobile roof supports were caught in the fall area. A large slip was laying at a slight angle along the #4 entry when the continuous miner was pillaring the left block the entry begin to give indications of failure.
Note: Date & Time fall occurred not known (is in worked out area) - Time noted was approx time fall found. An unintentional roof fall occurred on the 3 east north mains between crosscuts 4 & 5 in the No5 entry. The fall was approximately 60'long X 18' wide X 8'-10' high ( estimate). No injuries or property damage.
Roof fall was found during a pre-shift exam on MMU. 003-0 section. Roof fall is located in the #3 entry at crosscut #10 at survey station 6979 of 1-east submains. Roof fall is approx 18' wide x 7-10' thick and 60' in length. Roof fall is approx 30' feet inby and 30'feet outby crosscut #10. No injuries associated with this roof fall. The roof fall is located on a in-active section.
An additional ROOF FALL was discovered at a previous rock fall location on the #1 belt at crosscut #7. The original fall was reported in 2002. The roof falls combined measure 18 feet wide X 30 feet long X 3 feet in thickness.
A ROCK FALL was discovered two breaks inby survey station #1770 in the #5 entry of the 13th development panel during the weekly exam of the outby area. The immediate mine roof at the fall location was slate and the fall measured 30 feet long and 18 feet wide.
A roof fall occurred in the last open cross-cut, in the #5 entry, on the 003 section. The fall was approximately 25 feet long, 25 feet wide, and 6 feet thick. No persons or equipment involved.
DC loop contactor contacts arcing, burnt out bolt holding operating side of contact assembly. System ground faulted stopping the hoist. EE contacted the MSHA hotline. The mine was evacuated. A spare loop contactor was installed and the hoist resumed normal operations at 2:24 pm.
3rd shift hoist operator had the #1 hoist stop running-an electrician was called out. MSHA was notified at 4:50 am. The electrician determined the PLC needed rebooted. #2 and #3 hoists where available at all times, no evacuation was necessary.
An unplanned roof fall has occurred at brk. 39 on 1st N.W. Panel. The nearest spad to this location is #1322. Fall was approx. 17' high x 18' wide x 50' long, there was no injuries involved with the fall.
A alleged ignition of methane gas was described to have occurred in the face of the No. 7 entry. A flame was witnessed by the continuous miner operator while loading the third shuttle car in the cut. No injuries or damage to equipment resulted from this occurrence.
The #8 elevator was out of service for a period of time exceeding 15 minutes due to a power outage. Three individuals were on the elevator at the time. The power went off at 11:59 p.m. The power was restored and the EE's removed from the elevator at 12:40 a.m. on 5/24/11. All governmental agencies were notified.
Roof fall was discovered outby the #27 crosscut in the #4 entry on West Mains. The fall was approximately 10 to 12-ft long, 14-ft wide and 6-ft high. The areas was bolted to plan. This area also has water coming out of the roof which during winter month freezes which contributed to the deterioration of the roof.
During an inspection of the Hart Street Elevator by MSHA Inspector a damaged place was discovered on the govenor rope near the bottom sheave. The hoist was immediately taken out of service and the MSHA One Call center was notified.
The plant was shut down at 3:30 pm because of a rock in the impeller on the dredge. After this task, 2 miners were at the controls in the plant electrical trailer to start the plant back up at 4:00 pm. It was within a few seconds of pushing the slurry pump start button, that the 2 miners heard what sounded like an arcing sound & then a loud bang
A roof fall was discovered at the 4 O panel, entry #7, xc-22, spad 14+56. The approximate size of the fall was length 33ft, width 19ft, height above 6ft.
A CAT 16H Blade caught fire on the Pit 14 haul Road. The cause is believed to have been electrical but not determined at this time. The fire burned for more than 30 minutes but was extinguished by a water truck. No one was injured in the incident.
A roof fall occurred in the #9 seam of coal in the 2nd Southwest Mains at crosscut #20 in the #10 entry. The fall measured approximately 35'x18'x6'. The area was supported with 6' tension rebar bolts, 8' extension bolts and cribs.
The brake system on the hoist car at the Roadfork #51 mine malfunctioned for a period of time that exceeded 30 minutes. When examined it was found that the tips ""burned up"" on a BC contact. The contact was replaced which restored the proper function of the braking system. No injuries were reported.

A non injury Roof fall occurred in the #7 heading, The roof fall was reported to MSHA by Supt. of White cabin #9, EE was assigned by MSHA to investigate the roof fall. The roof fall measured 18'x18'x7'

Unintentional fall that occurred on HG5, #2 entry, break 8 in the intersection. The fall measured 19'W x 19'L x 10'H
At 1:34am the #1 coal mill was operating normally when the kiln operator received an alarm that one of the two explosion vents opened. Hot material was inside the mill inlet ductwork. The coal system was flooded with water.
Roof fall K-Butt Return #1 & #2 Entries approx. 4000' outby face. Roof Fall started at survey station #11732 #2 Entry and ended at station #11826.
Unintentional fall that occurred on HG5, #3 entry, break 11 1/2, in block. The fall measured 19'W x 56'L x 10'H
An unplanned roof fall has occurred in the #3 entry between crosscuts 16 and 17. (#5 belt). Fall was approx 10'thick x 15' wide x 55' long. Fell inby dumping point resulting in K-order and change of RCP. No injuries involoved and the fall did not affect ventilation or travel ways.
At 3:03am the #2 coal mill experienced an airlock which stopped coal feed to the coal mill. When the coal feed stopped the coal mill exit temperature began to slightly rise from 127 degrees F to 131 degrees F. Both rupture vents opened due to an ignition of coal dust in the system.
An Unplanned roof fall has occurred in the #6 entry between crosscuts 13 and 14. (#5belt) 4' to 6' thick X 17' wide X 30' long. timbered and dangered off 2 days prior to the roof fall, no injuries involved with fall and the fall did not affect ventilation of travel ways.
A fall of roof occurred in the Southwest Submain at crosscut # 41, entry # 2. The fall measured approximately 20 feet in length x 20 feet in width and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
5550 14 stope east experienced a rib failure on the south wall. The affected area is approximately 18 feet long. Approximately 6 feet of material sloughed off to the anchor point of the bolts in the affected area.
A fall of roof occurred in the Southwest Submains at crosscut 24, entry 2-1. The fall measured 40 feet in length x 12 feet wide x 3 feet thick. The area was supported with 6 foot fully grouted roof bolts.
An unplanned roof fall has occurred at brk. 7 of 4A belt in the #3 entry. The nearest spad to this location is #1592 approx. 100' outby the fall. Fall was approx. 14' high x 18' wide x 25' long. There was no injuries involved with fall and it did affect ventilation or travelways.
An unplanned roof fall has occurred at brk. 3 of 4A belt in the #1 entry. The nearest spad to this location is #1829. Fall was approx. 16' high x 18' wide x 25' long. There was no injuries involved and the return was re-routed around the fall area.
A malfunction in the electric powered air valve for the N gate of the production shaft skip pocket locked in the open position. Due to trouble-shooting and repair time, the mine was evacuated. No injuries or other problems. Hoist back in service at 5:57am.

AT APPROX. 1:00 AM THE EXAMINER FOUND A ROOF FALL IN # 1 ENTRY X-CUT # 141 OF THE MAIN WEST ENTRIES.
An unplanned roof fall has occurred at 34 brk. in the main return air course 40' outby the return overcast in the #2 entry at spad #1271. Air course was for both #1 and #2 sections the fall measured 17' high x 19' wide x 20' long there were no injuries involved.
Roof fall in number 2 entry (belt line) at intersection of x-cut number 61 of the Headgate Unit. The fall is 12 foot by 15 foot and fell out 8 foot high.
An unintentional roof fall occurred on the MMU 011 working section at station 7+35 Entry #8 intersection and the crosscut between Entry #7 and #8. The fall measured 50 feet x 19 feet x 9-10 feet high. The fall did not affect ventilation, passage of men or cause personal injury.
Power outage in Mullan where the mine is located. The power company had been notified and are working on the problem as I write this. Should be back up shortly.
A methane ignition occurred at No. 45 shield on the face of the S-2 longwall panel (MMU 034-0), as the shear was cutting to the headgate. The ignition occurred on the tailgate drum. There were no injuries.
#8 elevator was out of service for a period of time exceeding 30 min due to the car overshot the landing by 6"". Schindler was called the top level switch target adjusted and the buffer switch was reset. The car was tested and placed back into service @ 7:20pm. All necessary government agencies were notified according to law. No 1 was on the elevator at the time.
The #3 Coal Mill went down loaded due to a high mill exit temperature. Upon restart some 3 1/2 hours later and ignition occurred inside the mill circuit causing one of the coal mill vents to open.
An unplanned roof fall occurred in the #3 belt entry one cross cut in by SS 361 n the intersection. The fall was approximately 10' high - 28' wide (including area in adjacent crosscuts) and 3.5' long. These are approx. measurements. There were no injuries. The belt drive and tailpiece were under the fall and will not be recovered. Area old.

Slicken sided formation with laminated shale fell at or near anchorage zone 16'-17' L. X 16'-19' W. X 6'-6.5' Thick.
During routine maintenance of the #1 Hoist a fault occurred on the hoist control panel showing residual air pressure present. The residual air pressure system gauge read zero indicating the hoist system was operating correctly. In trouble shooting the control panel fault a relay base mount was found to be cracked and electricians replaced the base and cleared the fault.
Bad roof area, 3 North Butt, #27 Room - left side. Affecting travel but not airflow.
An unintentional roof fall occurred on the 2 South 1 Entry 6, Crosscut 10. The roof fall measured approximately 40' in length, 18' in width, and 7'-8' in thickness. A 103 (k) Order (8434030), was issued May 11, 2011, by MSHA Inspector, Phillip D. Herndon.
A methane Ignition occured in the number 1 entry on the west main left section as the roof bolters was installating roof bolts. As the roof bolter went to tighten the roof bolt he saw a flame from around the roof bolt plate.
Severe thunderstorms were moving through the area of Deep Mine #35 on the evening of May 10th 2011. During on of these storms a bolt of lightening struck the mine office causing a fire at the mine office, with area wide power outages and no phone communications. The mine was evacuated and there were no injuries to any of the mine employees related to this incident.
Two employees got trapped on the elevator at approximately 5:11 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The employees were freed at approximately 6:15 AM. The elevator was throughly examined and evaluated. The elevator was returned to normal service at 10:30 AM.
Roof fall on D-Belt in #5 entry between cross cuts 11-13, approx. 8' high. Fall was posted and dangered off.
A fire did occur on mine property involving an over turned Kenworth Tandem coal truck. The fire lasted for approximately one (1) hour before being extinguished. There was no personal injury involved with this accident.
An unplanned roof fall occurred in the intersection of the 12th crosscut of the #4 entry of the north mains. The fall was approximately 18'wide X 20'long and 8' to 10'high. There was no property damage or injuries. The fall will be cleaned up. This is an out by area along the main travel way.
When adding extra water from the City and Plant, in an effort to fill pond three for the first time. It was believed that the additional water being pumped into pond one overflowed and washed out the South East corner of the Pond. No water entered the Plant or Mining areas, the water traveled East across Exel Easement into a wooded area and a farmers field.
A roof fall was discovered in the 3rd North East Sub-Main, between entries #9 and #10, xc-11, spad# 9+10. The approximate size of the fall was length 19ft, width 16ft, height approx 6ft.
A fall was discovered at B-2 from #3 Heading up to and including #5 Crosscut and outby on #4 Heading to #2 Crosscut. The fall occurred 1000' outby the face, and the area was bolted with 6' Super Twist bolts and 12' cable bolts. This made IE 2B2 inaccessible and blocked an alternate excapeway. A new escapeway was established.
4160 cable was hit with excavator, knocking out the power to the # 3 hoist.
Production hoist kicked out. The production hoist and Production Shaft are the second escapeway from the mine. Mine was evacuated. A limit switch on the south side of the Production Shaft shorted out. It was replaced and the hoist became operational again.
BC #1 Mine had an unplanned roof fall in the #9 cross cut between entries #5 & #6 on the left mains section.
At 2:15pm the weekly examiner discovered an unintentional roof fall in the 8 South Bleeders on the 8N panel in the #1 entry at #40 crosscut. The fall was approximately 18'W x 120'L x 6'H.
As the miner operator was loading his last car on the left side of a blow through he noticed a pile of coal in the cut was possibly burning. It was determined through company and MSHA investigation that a methane bleeder from the mine floor and sparks from the miner bits is what possibly caused the ignition. No one was injured and no equipment was damaged.
An unplanned roof fall occurred on the 1 South belt at x-cut #9. The fall was about 6 to 8 ft high, 20 ft wide, and 20 ft long. The area was bolted with 5 ft resin bolts. The fall will be cleaned up and rebolted, by the clean up plan.
A parish approved control burn had been conducted when a small secluded area in the mine away from mining activities became ignited. The fire consisted of only brush being burned in a 2 acre area. The fire was extinguished in 1 hour and 1 minute.
During a routine check, a guide cable was discovered to be loose. All miners were evacuated using main slope. Maintenance employees repaired the cable. Miners then returned underground.
An unintentional roof fall occurred on the 1 North Main West 1, Entry 7, Crosscut 103 Last Open Crosscut. The roof fall measured approximately 30'-35' in length, 22' in width, and 8'-10' in thickness. A 103 (k) Order (8433535), was issued May 05, 2011, by MSHA Inspector
A non-injury roof falls occurred in the out-by return air course. The roof falls were discovered during the weekly exam of the return air course. The falls was approximately 50 ft. long x 19 ft. wide x 6 ft. in thickness, and 12 ft. long x 12 ft. wide x 5 ft. in thickness.
Roof fall at face area #2 entry, 45' to 55' long, a-mains
An unintentional roof fall was discovered in front of the #1 seal of the 14 seal area. Size of the fall was estimated to be 20' L. X 20' W. X 6' Thick. No injuries, ventilation was not interrupted and no equipment was involved. The fall will be cleaned up.
The hoist elevator at the Enlow Fork Mine #1 portal was out of service for more than 30 minutes. The programming system was not communicating with the hoist to allow the elevator to move up or down the shaft. The P.O.C. was reprogrammed and the evelator put back in service. No one was on the elevator at the time of incident. No injuries.
A roof fall occurred in the #2 entry 1 block inby spad #2942 in an intersection. The fall was 6' high x 17' long x 17' wide. There were no witnesses nor equipment involved. The area was breakered off on all 4 sides. It will not be cleaned up. This was outby the section tailpiece.
A reportable roof fall was found in E3 startline, entry 1, crosscut 3. The fall was approximately 30 feet long, 20 feet wide and 14 feet high.
An unintentional fall of the mine roof was discovered at the North West Mains panel, intersection at spad #993, #2 entry. The fall measured 20ft. x 20ft. x 8ft. The fall was immediately reported to MSHA and the State.
An Unplanned roof fall occurred in the road fork #51 mine that measured approximately 45'long X 20'wide X 6'high. No injuries were reported. The area was developed on 3/15/08
Elevator #1 was out of service for a period of time exceeding 30 min due to a noise coming from the deflection shield. Elevator was taken out of service on 5/3/11 11:20pm, placed back into service 10:15pm on 5/9/11. No one was on the elevator at the time. All necessary agencies were notified according to law. Escape ways were re-routed.
An unplanned roof fall has occurred at spad #3916 on the Arabian Mains #6 entry. The roof fall was 20' x 20' x 5'. No injuries involved, no equipment involved or passage blocked.
A roof fall was found during a weekly exam in the 1st Left panel off south east mains 3200' outby the 001 section. The fall measured 420' wide, 19' wide, and 8' thick. The fall occurred above 6' torque tension roof bolts due to coal and shale laminations. The area was developed 10/31/09. No injuries occurred. Ventilation, passage of men, and use of equipment was not impeded.
While attempting to connect 3 north mains to 2 east panel 4 for the purposes of pumping water from worked out panel an inundation occurred. The amount of water encountered at cut through was greater than expected & the result of the inflow typically was from a feathers edge to 6""-12"" deep with the max. being approx 30"" in a swag in belt entry. No injuries or entrapment occurred.
A roof fall was discovered in the 4 O panel, # 3 entry, XC-19, spad #12+64. The approximate size of the fall was length 25ft, width 18ft, and height 6ft.
A roof fall occurred at in Entry #5 (belt entry)in the North Submain of the Main West.
An unintentional roof fall was discovered at 18 break along the # 6 belt in the reurn in # 3 and # 4 Intersections. Size of the fall was 4' to 5' thick the area is dangered off. No injuries, ventilation was not disturbed and no equipment was involved. The fall will not be cleaned up at this time.
Kiln 1 coal mill experienced a minor unplanned ignition of coal dust at 4:36am following a shut-down. No damage or injury occurred as a result of the ignition. MSHA hotline was contacted at 4:40am.
Elevator was down for more than 30 minutes due to an electrical problem.
The mine roof in crosscut 25 on the east side of the North A track fell above the anchorage of the existing roof bolts. The affected area is approximately 25 feet in length and 18 feet in width.
The 8 Haulage elevator was down for more than 30 minutes due to a brake problem.
992 G end loader counterweight/fuel tank fell suddenly, pinning employee against the ground.
Employee placed his left hand on the outside of the canopy of the scoop that he was operating. The scoop went into a mud hole and slid into a rut smashing his left hand between rib and scoop canopy. After surgery, Dr. said that he would lose 1/2 of his left ring finger and tip of his middle finger.
EE & coworker move a CM to the mine bottom. A crib had been placed under stab jack & was pressurized to raise the miner up. As the pressure was removed form stab jack inj ee attempted to remove the crib block but it was wedged beneath miner. Stab jack was again pressurized causing inj ee to catch index finger between the crib block & the bumper of the miner.
Employee was operating the Joy 7LS long wall shearer. A piece of rock approximately 2' long X 1' Wide X 6"" thick had become lodged in the bretby trough. EE lifted the rock out of the bretby trough and attempted to shove it over into the panline. While doing so the rock slipped and fell back onto his hand amputating the tip of his left ring finger.
EE was operating a HDPE fusion machine fusing two 16"" HDPE pipes. While loading the machine, the section of pipe jammed on the saddle not allowing the pipe to feed properly. The ee attempted to reposition the pipe by hand leaving his left hand exposed in a pinch point. The pipe came loose and went forward pinching his #2 and #3 digits on his left hand.
Employee was operating shuttlecar when he reached out of the canopy to move a CO monitor line. He caught his right index finger between the shuttlecar canopy and the mine roof.
Air Conditioner repairman (NOT A POUNDING MILL QUARRY EMPLOYEE) caught his left hand. Last joint of middle finger was severed.
Miner was checking the saw belt while saw WAS NOT running. His hand slipped and hand fell onto saw blade and cut his right thumb and the knuckle. Miner was taken to Hendricks Hospital in Abilene Texas and was treated. Miner was released the next day. Miner should have been more careful where he placed his hands to check belt in future.
Employee was placing 105 lb. Spreader Bar & Dump Block pins into pin hole and the pin slid into place faster than expected crushing employee's left tip of ring finger between the pin head and the dump block..
Employee was removing screen material when the screen shifted and he grabbed it resulting in the amputation of the tip of his right index finger.
Employee bent over to pick up a 3C/#8 electrical wire end to roll up on truck and felt a twinge in the left hip.
The truck operator was being loaded and the loader bucket struck the bed of the truck. This jolted the employee and caused a muscle strain to his upper back.
Employee was reclaiming curtains and picking up trash in the left side Entry 2 when a loose rock fell from the top striking the employee's arm. Injuries resulted as an abrasion and strain/sprain to his left arm. Employee will remain off work until released by the doctor on June 06, 2011.
Miner reported knee pain of a chronic nature occurring over several weeks that culminated in a stiff leg a popping sensation, pain & a warm burning to his knee as he exited a vehicle. There was no specific event that preceded this occurrence & injury appears to be of a chronic long term nature. Employee is off work until he can be evaluated by his personal physician
Employee struck rock in the roadway with scoop and hit his head on the scoop canopy; resulting in strain to the muscles in his neck.
The injury report was received by fax on June 16/11. - Late report - The injured was operating an LHD & trying to pick up a large rock. The LHD slipped over the rock causing him to jar his back against the seat. Note: Worker has been off since June 1/11.
Employee was walking in the return entry looking for a step ladder and stepped onto a rock in the roadway twisting his left knee. Employee was seen by a doctor and released after the accident and continued to work. Employee went to another doctor on 07/18/11 for pain in left knee and was taken off work at that time pending the results of MRI. First day off work 07/19/11
EE was riding mantrip with the crew to #2 section. He was riding with the operator instead of the passenger compartment. The mantrip was not over crowded. EE said he was just riding there. He reached for his tobacco and his head hit the mine roof causing neck pain. He was released from CAMC hospital and told to remain off work a couple of days to rest his neck.
Employee got out of his pick-up truck, turned in the open doorway to retrieve his respirator and while doing so the wind blew the door shut, forcing the employee's left knee to impact the truck body.
EE reported to me that on 5/31/11 he strained his back while bagging. He reported this on 6/2/11. He was sent to the medical center for evaluation. He is scheduled for massage therapy with ice and medication. EE is to be reevaluated 3-4 days.
Employee states that he was getting down off a forklift and when he put his left foot down on the floor he heard a pop. Later that night his foot began to swell so he went to the emergency roo m where an x-ray revealed two fractured bones near the joint where his small toe joins the foot.
The Injured employee said he slipped in a mud hole on the section and strained his knee.
Employee states he got rock dust in his eye while freeing up the rock buster. He also states he was wearing his safety glasses.
A contractor employee was assisting in installing water line when the water line fell striking him under the chin causing him to fall backward into the Sandvik hauler canopy. This resulted in a laceration to the chin and back of the head requiring stitches.
As Employee was carrying a sand jack he tripped and fell. The sand jack fell on Employee and struck him in his lower back. Employee was treated at Raleigh General Hospital.
While shoveling the 5C head his shovel got caught in belt and kicked back. When the shovel kicked back it dislocated pointing finger on his right hand and tore the tendon. Employee continued to work until 7/11/11 when he went off work for surgery on his finger.
Employee was walking between a coal rib and his battery ram car. He slipped on a wind row of loose fire clay and twisted his left knee and ankle. He fell to the floor and rolled his left ankle resulting in a fracture to his left ankle.

The employee (a Mine Examiner) states he was walking on the back side of the 4th North belt when he tripped over a water line coupling, fell, and struck his forehead against the belt structure. The injury required 12 sutures.

Was operating a haul unit in mine transporting a load of rock from the face to the primary crusher. Drove over 2 dips in the road causing the seat to bounce and allowing ee to strike his head on cab. Ordinal diagnosis was neck strain and returned with no restriction. EE continued med evaluation & was seen by a specialist on 7/14/11 with recommendation for 3-4 PT treatment.
Employee was operating the #3 Fletcher Roof Bolter (Double Boom); while attempting to dislodge steel from drill chuck he missed the steel and struck the drill chuck with his claw hammer causing a piece of steel to fly up striking his eye. Injuries resulted in a laceration to his eye which lead to surgery and will be off work on Workmans Comp for an uncertain period of time.
Trigger was engaged when plugged in the cup grinder fell to the ground and the cup shattered and a piece hit the EE in the leg breaking the bone.
The injured was bolting in the 7 right crosscut. After he stacked his pusher and finishing piece of steel together and was drilling to his required depth, he went to get a bit out of the tray to change on his starter steel and a piece of rock fell on top of his canopy, and continued to fall on his left hand.
Employee was cleaning the tailgate area of his dump trailer when his second and third finger of his right hand got caught resulting in two fractured fingers.

The employee was removing a wheel from a fuel truck with an impact wrench when the socket broke causing a piece of the socket to strike him in the abdomen. He treated the wound himself and continued working until May 30, 2011. On that date, he was admitted to the hospital with infection in the wound.
Employee was opening a bucket of plaster and some of the plaster splashed into his left eye. Employee says he was wearing safety goggles, management does not agree. Employee was transported to Summersville Regional Hospital and given time off from work.

Employee was helping to install a pressure gauge in 8"" aqua pipe when the pipe cracked and busted and knocked the employee to the ground causing multiple injuries including a laceration to the head.
EE picked up a bag of rock dust, twisted to put on Boss Hoss. Alleged pain in his lower back. E-Doc #14116925 that was submitted 5/10/11 for alleged back injury on 4/28/11 was determined by Workers Compensation (BrickStreet) to have sufficient inconsistencies and contradictions to concluded that he was not injured in the course of and result of employment.
Employee was walking next to the Unit #3 travel road. A rock fell out between roof bolts and struck him on the back of his lower right leg. The rock was approximately 24in. x 24in. x 3in.
While bending over to pick something up from the ground the EE strained his lower back.

While placing a metal plate onto coal hopper to be welded the miners left little finger was pinned between metal plate and coal hopper.

The driver of the mine water truck had stopped in the middle of the road to advise the GSSC patrol vehicle that there was brush on the front of the vehicle. The GSSC patrol officer was looking at another area when he returned his eyes forward he had struck the rear of the water truck.
A manual grease gun with a 15"" hose failed while the employee was greasing the wheel loader. When the hose failed, grease shot into his left eye.
Haul truck drove though a rough spot on haul road, jarring the operator. Pain meds were prescribed.

EE was operating electric locomotive with 9 ore cars attached when he went thru a closed air door, taking out the air door with injuries which included three broken ribs, punctured lung, laceration to back of head.

EE stepped in wet cement during a cementing operation at the site of a well plugging operation 5/20/11. A chemical burn developed over the weekend on his left foot. The EE sought medical attention on 5/23/11.

Worker was cleaning screens at Crusher & lifting rock that is on crusher screens over rail into loader bucket. Worker thought this was an old 2007 injury & WC claim when he went to dr. This incident is currently under review & being treated as an aggravation to a pre-existing injury. We are currently waiting for confirmation from the dr & WC if it's a pre-existing or new injury.
Employee was cutting coal toward the longwall tailgate when a rock fell off the shear plow hitting employee on his right side and landing on his right ankle.
Employee shoveling belt when supervisor noticed employee in pain and lowered to his knees. Employee stated he had prior back issue and pain level was a 8 - he was removed from the mine for treatment.

A haul truck operator let go of the steering wheel to adjust the seat head rest. The truck drifted off the road and climbed up on a bank, then turned on its side. The operator received a laceration on her forehead and soft tissue injuries. Due to pain medication and soreness, the operator was unable to report to her next scheduled shift.
EE was walking down stairs when he slipped and fell down last 3 steps.
Setting up camera, tripped over rock, fell, tried to hold self from falling with hand, hurt wrist.
17-W left long-wall set up face, Individual was attempting to reposition a nylon scoop rope when his left hand was caught between the scoop blade and wrench rope. Individual suffered a fracture to the left index finger & middle finger, received 9 sutures.
The employee went to get a section of vent bag and as he was loading it onto the buggie, he pulled on the vent bag and it got caught. When the employee pulled on the vent bag again, his back popped right below his shoulder blade.
Employee was struck in the face by a pnuematic/hydraulic ram while working on the 645 Haul Truck. Employee received stitches to nose and lost 4 teeth.
On 6/2011 employee went to the Dr. asking for short term disability for neck and back pain with no mention of a work related injury. On August 22, 2011 when the short term disability was running out, he reported a work related injury to HR. HR investigated and determined there was no evidence to support a work related injury.
Injured member was lifting supplemental roof support timbers when he twisted his left knee. He did not cease work and continued to work his regularly scheduled shifts until May 25, 2011. Due to continued discomfort, injured was evaluated and excused from work by his doctor and awaiting an MRI on his knee.
Employee stated that while getting on the lozer, he had 2 hands on the handrail and 1 foot on the ladder, while pulling himself up he heard a pop in his right arm.
Employee hit shoulder on roof bolter canopy & fractured shoulder. Started missing work 5-17-2011.
Employee was getting off of super steer when he slipped and fell causing him to twist his left knee.
Employee was shoveling loose coal from mine floor onto 4-A belt conveyor line at number 6 break when a piece of coal rolled from the inby corner, on the walkway side of the belt. It struck him on the left leg just above the ankle.
Member was unloading 4 wide by 8 long pallets made out of 2 by 6s with 4 runners on the bottom off of a flat car into a scoop bucket. While doing so he experienced sharp pain in his lower back causing him to go to his knees with the pain. He took himself to Summersville Medical Center Emergency Room for treatment.
Injured member was stepping off a haulage car when he slipped in the mud causing his legs to split and injuring his left hip. He was diagnosed with a muscle sprain/strain and excused from work.
Employee was getting off loader when his left foot hung in the ladder step pulling left knee and muscle. Employee required surgery on June 2nd and began losing time.
Individual was working at the 5 West #1 Transfer removing material that accumulated along the belt when he was lifting up material felt pain in lower back. Individual continued to work until 5-17-11 when he went off work.
Employee was sitting in a low trac when a rock fell out between two pins striking employee on the right arm and shoulder. He returned to work with out restrictions his next scheduled shift. He sought orthopedic consult on 8-5-11. He started physical therapy and was unable to return to work do to restrictions. Notified by workmans comp that his claim had been approved on 8-18-11.
While mining in the number 3 heading of 14 Left, approximately 160' in the cut, the employee stated that he had placed a roof strap on top of one of the jacks of the miner. While backing the miner up to correctly position the roofbolter, the strap fell from the jack and struck the employee's right hand.
Injured was pulling mesh over ATRS and mesh hung on ATRS and he jerked it and hurt his shoulder. he returned to work on 5/14/11, but then left on 5/16/11 with pain in his shoulder. First missed day was 5/17/11
While hanging curtain, injured fell off of ladder injuring shoulder.
Employee was in the process of getting the bendictor cleaned at eh #9 head. He shut the #8 belt off by the switch. He the got into the rock box, at which time the belt started again with him on it. Employee grabbed a belt chain which threw him off the belt. He was transported by ambulance to Summersville Hospital, received stitches and treatment for fractured ribs.
Unepected release of hot clinker from a kiln.Packing was blown out from around a burn pipe.The hot clinker struck our employee on the back of the head,neck,shoulders,and back causing second degree burns.
While ee was installing a conveyor belt, EE was using a cutting torch to remove bolts, the torch somehow got entangled in sleeve of leather welding jacket causing torch head to pull into his sleeve causing second degree burn to his left forearm.
Employee was working to remove the teeth from an excavator bucket. After cutting the teeth from the bucket EE used a sledge hammer to dislodge the teeth. While pounding the teeth off the bucket a piece of metal broke from the hammer striking his leg. EE was taken by ambulance to Western Maryland Health Systems where he received 5 stitches to repair the laceration.
Employee was cleaning 2nd floor of plant when a piece of 4"" piece of metal fell from 3rd floor where it was being cleaned hit him on his right shoulder. Bruising it.
Employee got out of shuttle car to pick up a bundle of volcanoes plates, when he slipped on sloughage causing him to fall forward. This resulted in him pinching his left thumb between the mine floor and the volcanoes plates.
Employee received a lime burn on his left wrist and arm. Employee was wearing the proper PPE including glasses, dust mask, gloves, and tyvek suit.
EE was getting metal roof straps from the supply area. He cut the metal band holding the straps together and the end of the band cut his hand. This cut became infected, specialist took EE off work 5/16/2011.
A rock measuring 3'x 2'x 0"" to 5"" thick unknowingly sat down on the roof bolter canopy. While he was swinging the drill head over to install the 2nd bolt, he saw the rock coming off the top of the canopy. While moving out of the way of the falling rock, he hit his head on the mine roof causing pain to his neck and shoulders, he began losing time away from work on 05/17/2011.
EE was in the process of stacking 100lb. bags on to pallets. He was completing stacking bags on the last pallet of his shift when he felt a pain in his left shoulder. He reported that he felt fatigued and the product was coming faster than he could handle.
Employee was operating a Mack Coal truck when the truck struck a pot hole. The air ride seat lost air causing the employees seat to bottom out, injuring his lower back. The employee was placed off work due to a diagnosis of bulging disk's.
The man reported that he was walking to the feeder when he felt pain in his low back.
Employee was hitting side of dryer with a hammer when hot material came out of thermal coal dryer next to him.
Employee tripped over a hose and fell to the ground. He was installing a suction line on a diesel pump. Incident was reported but employee said he was OK. Several weeks later employee complained of shoulder pain and was taken to the doctor when a torn rotator cuff was discovered. After several weeks it was determined that surgery was finally warranted.
The employee was walking by the boom truck when the hook struck him on the right side in the ribs.
Employee was kneeling on the grip strut catwalk, on a glove, while installing a return idler guard on under side of conveyor.
An accident occurred when a shot of 37 holes was detonated in the pit. The employee was properly contained inside a steel blast shelter at the time of the detonation. However, the blast shelter was improperly placed and rolled on its side following the detonation. The employee suffered 3 broken ribs, a collapsed lung, and a broken bone in his neck and on his left ear.
This operator took another out to the dredge by jon boat. His return trip was abruptly stopped by a sand bar. He lunged forward and caught his foot in a bottom boat bow, wrenching his back. Sent to doctor for lumbar strain, muscle spasms. Doctor placed him on modified duty. Tough to not reach or stoop or bend on any sand plant job. He stays at home and gets physical therapy.
Material buildup against clean out door, pulled clean out door pin w/pry bar. Pressure release against door threw pry bar. Hit worker on right side of head, scalp cut above ear just below hard hat. Stitches required to close wound, no other damage, no impairment.
The worker was walking through the intersection of #3 and #4 entry, stepped into a hole and twisted his knee. He worked with no restrictions until Sept. 5, 2011 when it was decided to have his damaged knee corrected.
While removing a bearing from a floatation agitator with a dead blow hammer, the bearing broke free and struck the employee in the back of his right hand. It caused a laceration, approximately 1/2 inch in length. It was cleaned and bandaged, however, it became infected. The employee was sent to the clinic, had the wound cleaned, which lead to the discovery of a damaged tendon.
When holding metal up to be welded in place is when the arc flash occurred.
EE was dragging and hanging 12470 hi-line above the head. EE felt pain between the shoulder blades. EE continued to work until 5/26/11 when he went off work.
Employee was attempting to close the outby set of doors. The inby set and one of the outby doors were already closed and was closing the second door when his hand was smashed between the outby set of doors. Injuries resulted in lacerations and fractures to his hand.
Employee fell at the 801 booster and hurt his shoulder. MRI and an Ortho Consult was requested. On 5/25/2011, the employee was given No Work Status by the Ortho Consult. Employee notified FMMO safety on 5/31/2011.
EE stated, I pulled a muscle in lower back, picking up 55 gal. drum. Emptying into a Bob-Cat bucket. Used hands to almost empty barrel. Some scrap metal left in barrel , lifted barrel and heard popping noise in lower back.
Employee bent over to pick up a block when he felt a pop in his lower back.
Employee was learning how to roof bolt. He was installing the second bolt of the row, Pushing the bolt to the roof. The hole was not straight and the roof began to bend. At this time the yellow extension wrench snapped and struck employee below right eye.
Employee was in process of being task trained to operator 330 Articulation truck and was a passenager, loader 950 got into soft spot and loaded landed on its side at the loading area. The loader bucket struck the bumper of truck.
Employee had bent an eight foot resin bolt due to seam height. The roof bolt would not seat in drill chuck. He hit rotation and the roof bolt struck him in the right wrist resulting in a fracture.
As the driver was traveling out of the pit and onto the south haul road, the driver did not see a pot hole and drove his truck through the hole injuring his left neck and shoulder.
Stepping off platform, misjudged distance, fell forward landing hard on right foot. Sustained sprain of foot. Incident was reclassified to Recordable Lost time on 5/25/2011, when restrictions could not be accommodated.
The crew was moving power up. EE was pulling Crawler cable slack, when he pulled something in his back.
Member was walking to return shaft when he fell forward into a small waterhole that was 2"" to 3"" deep he continued to work until 11;30 pm when he stated he might need to go to hospital, he was placed off work for 4 days because of soreness and swelling in his knees
EE was changing out suction hose on the roof bolter he put his left hand over pot to check suction when his right hand slipped hitting joy stick allowing pot to raise and pinch left hand between pot and drill steel. It fractured top of his left hand.
While lifting felt slight sting and got sore in lower back
Tryed to move a rock using his hands when another rock fell against his finger.
He was working on miner when he heard the top working. He jerked up quickly and felt a sharp pain in right hip and lower back. *** He began missing work on 5/11/2011.
The injured was operating the bolter to install a fly board for the face of #1. Curtain was hung across the intersection to keep air in the face. After he drilled his hole and was pulling the steel down he used his hand to help bring it out and the air blew the curtain into the driller station catching the steel and wrapping his arm into the steel.
Two employees were carrying a section of drill casing to a drill rig by hand. One of the employees slipped in the muddy conditions and dropped his end of the pipe. This put strain on the other employee all of the sudden and he stated that he experienced pain in his lower right back area and his upper left shoulder area.
Employee alleges back pain when working on drill. Employee did not report to employer till June 10, 2011
Jumped off screen deck to floor by stairway he sprained left ankle.
Changing middle deck screen on JCI 7x20 screen. Moved outside the rail protected catwalk and stood on screen I-Beam frame to throw screen to ground. Was not wearing fall protection harness. When threw the screen, the screen caught his hand or glove and pulled him to the ground 8 feet below, causing two fractured wrists and fractured ankle and a bloody nose.
Employee reported a swollen right knee near the end of his shift on 5/10/2011. He had been performing electrical work and had to work on his knees (with knee pads) for periods of time. He was diagnosed with bursitis.
The employee stepped through a man door and hit his head on a 2"" pipe. Employee didn't request treatment until next shift 5/11/2011.
While helping electricians pull a cable, the cable broke and the pulley swung and hit the employee in the leg.
At about 10:30am ee stated that a small piece of draw rock pushed him to the ground and hurt his neck and back. He said he was not and could not go to the emergency room. After driving his self home he called me and said his doctor said he could go.
While operator was performing a splitting operation, a slab of stone fell and caught the operator's left arm causing an abrasion.
After parking, employee got out of his vehicle and rolled his ankle.
Employee stated that he was walking bent over and struck a bent roof bolt plate causing pain on his right side of head and neck and busing hard hat.
Hot raw feed spilled from inlet to kiln. The feed came in contact with a wooden pallet which caught fire. Employee stepped in pile of raw feed which he did not believe to be hot. Some got down back of his shoe on left foot causing burn to skin.
On April 11,2011 while moving rocks out of the way of the bolter EE twisted his back. EE went to chiropractor had an adjustment did not miss work. EE's back did not get better had to get a MRI then had to have surgery on his back on 5/9/11.
Employee was running a Caterpillar D10R Dozer. As he was backing up, he struck a 3' high pile of coal which gave him a jolt. Employee claims he began to see a black spot in his right eye.
While shrink wrapping a pallet of stone the employee bent over and pulled a muscle in his lower back.
Employee had just finished his shift and had parked the cat scraper he was operating. As he stepped down off the last step his foot got hung up in the last strap causing him to stagger back twisting his right knee.
Employee reached over in the crusher to pull out a Rock. He felt his Left Shoulder pop which caused his arm to go numb.
Employee jumped off a piece of equipment and turned his ankle.
Crew was moving rail with pull straps (4 men) employee fell and pulled rail onto his right foot.
Employee was riding in a jeep when it collided with a locomotive. This caused a strained neck, back and torn shoulder muscle.
Ee was walking toward face from the mantrip when he fell and hurt his knee. The doctors are saying that surgery is needed due to torn ligaments. Ee will be off work for a few weeks.
Employee states that while putting up rib boards, the bolt spacer fell from the T-Bar striking him in the right shoulder and neck. He also states that it knocked him to the ground.
The operator was jarred while being loaded. This injury occurred on 05/08/2011 but did not begin losing time until 05/24/2012. (1 year later)
Injured employee was replacing a damaged high pressure transducer on #124 shield on the set-up face. He thought the hydraulic pressure was off because the fitting was not leaking emulsion. When the staple from the fitting was removed the fitting struck the employee on the left index finger under pressure. This resulted in a fracture and laceration to the finger.
The contractor employee was kneeling on the section conveyor belt at the tailpiece to foam between the feeder breaker and the tailpiece when the belt was turned on from an outby location. The employee was knocked face down onto the belt as he went thru the box check and then thru the belt transfer point. EE sustained 3 fractured ribs.
Employee stated that he was picking up a 5 gallon bucket of oil and felt a pop in his shoulder. Employee continued to work until the doctor placed him off work on 7/6/2011 due to a shoulder strain.
EE was dropping hammers off at our C-conationer located at Vulcan Pit in Knippa, TX. Pulled on a drill hammer to get it out of pickup and felt a pop in his back. He finished unloading the hammers and returned home. Rested that day and woke up the next day with back hurting. Tried to work on the 9th. Could only make 1/2 day before he went to doctor. No witness.

Employee was laying track in the track entry & lifting ties. EE went to the hospital for x-rays. X-rays showed nothing. Was sent home. Doctor said EE had a muscle spasm & came back to work with a no lost time accident. EE worked until 5/27/2011. At that time EE returned to the doctor again & the doctor at that time put him off work until further tests could be run.
Employee was trying to open a stone that was leaning against a block, the stone slipped and hit the worker in the foot.
As employee waited for Roofbolter to advance, a rock fell bouncing off the canopy and striking him on the shoulder, upper back and head.
While jackhammering #2 kiln, equipment kept braking and having to jump in and out of bobcat put stress on my right knee. Long periods of running bobcat stressed right knee, at which point I was unable to put any weight back on my right knee.
Spotting headgate shearer drum bits rock fell near tip of shield bouncing off of shearer drum pushing his head down on drum causing him to bite tongue and put strain on neck. Compliances rules/regs not a factor, PPE was not a factor, mining equipment/system was not a factor, miner profiency was a factor.
While operating the shuttle car near #3 intersection near spad 3106, member struck his head on canopy when he ran over something. There were no witnesses, member was taking by EMTs on back board to local hospital where evaluation showed contusion on lower spine.
Employee was moving miner cable out of roadway when she twisted her lower back.
Felt neck pain while operating shuttle car. Did not hit head. Was ruled compensable and mine notified on 10/6/11.
Injured was pushing dirt and rock with a dozer, dozer ran on a rock, dozer hit the ground solid, when dozer came off rock. Injured cracked rib on left side. This accident was reported to management on 5/12/11.
Employee stated that he was turning on fuel tank valve when he slipped and fell. Employee's first lost work day occurred on July 1, 2011 due to surgery.
Employee was pushing a coal hauler with a coal hauler, traveled through a hole in the mine floor causing operator to hit his head on the canopy.
Employee stated that while building an overcast, a beam started to fall and he caught it and strained his stomach.
Employee was plugging up an air greaser when he felt a pain and pop in left elbow.
Employee was changing bits on the tailgate side shear drum on the E-22 longwall section. Rolled drum and his left ring finger was caught between the bit ring lug and the face. Resulted in 10 sutures.
EE was walking to the section transformer at the start of his shift. The EE stepped in mud on the mine floor and into a hole and twisted his knee.
Miner was adjusting load on supply car when he slipped and sprained his right ankle.
EE stated that on the 14 Left section while lifting the miner cable and water line to place it in a cable hanger he felt pain in his, back radiating to his legs and groin area. The incident was not deemed reportable until 5/19/11 due to availability of medical records concerning a previous back injury.
EE was operating unit #1882 backhoe, EE stepped off the machine when he was getting off the backhoe his foot slipped off of the step and he twisted his knee. At that time the EE refused medical treatment and cont. to work. His knee began to hurt him more and he wert to doctor and has continued to work. Will have surgery on 2/6/12 and will be off 4 to 6 weeks.
Employee was using a wrench, the wrench slipped off the bolt, he cut his left hand when it struck a piece of metal.
Employee was adjusting the shaft nut of the cone crusher, he missed a swing with the hammer and struck right boot at steel toe protection area, steel toe caused pressure in toes causing swelling.
Employee was walking around the bottom of tower 2. Stepped on something and fell to knee, back started hurting. Went to 1A conveyor and was changing two return rollers. Back started hurting worse.
The truck driver said a load of material was dropped into the bed of his truck too hard by the loader operator and it jarred his neck.

Employee was operating shuttle car when he hit a hole in the roadway it bounced him up and hit his head on the canopy of shuttle car. Hurt his neck.
Employee reported that he stepped off a mantrip into water and stepped on a piece of rock or coal and twisted his right ankle.

EE was driving a loaded shuttle car back to the feeder as he turned into the belt entry, he struck the coal pillar. As he struck the rib he had his arm resting on the side of the cab. His upper and lower arm was pinched between the cab and rib. This caused a great deal of soft tissue injuries.

Employee was helping change a coal hauler battery. He was pulling the charging lead when the cable get hung up, pulling the employee around. He felt a pain in his groin area. The employee continued to work until 09/06/2011
Power knocked on CM (JM6432). Operator and scoop man were tightening the waterline swivel near a damaged place in the cable. Someone reset the circuit breaker and cable went phase to phase creating an arc. The scoop operator may have received. a shock when the arc occurred as he stumbled away from the cable. EE was kept in the hospital two days for observation.
EE was doing inventory when a truck pulled up and the EE turned around then twisting her left knee.

A dragline operator was observing a winch cable being pulled by a nylon rope when the nylon rope broke. The elastic force of the nylon rope struck him from the left side knocking him to the ground.
Employee was operating a motor when it lost power. Employee set brake but it failed; hitting inby doors and block causing them to hit employee. Employee didn't miss work until 05/12/2011.
Employee reported hitting a depression in a haul road on the stockpile, and the seat bottomed out as well. He did not notice immediate pain, but about an hour later, contacted his supervisor wanting to be examined. The ER physician took him off work for his last shift. On his next scheduled shift, he called in saying he needed more medical attention. ON GOING
Loading eight foot cable bolts on to the continuous miner and stepped in a hole spraining left ankle.
Employee was helping recover ventilation tubing in the return when a clevis to a chain that was holding water pipe in place broke hitting employee about left ankle.
Pulling on super stucco hose on a long wall power move, the hose slipped out of hand and the end hose fitting hit left foot above meta-tarsal on boot. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.
I WAS LIFTING A LARGE OXYGEN TANK ON TO THE OLD WELD TRUCK WHEN LEFT TWO POPS IN MY LOWER BACK.
Welder was descending a set of stairsteps. His foot (had crater on bottom of boot) slipped on a strip of concrete/metal, causing him to catch himself using his right hand/shoulder. The welder strained his right shoulder.
The employee's left hand got smashed because he went to remove a rock from the tailgate, and the tailgate smashed down on his left hand, resulting in a fracture of finger #4. He was issued restrictions, in which we were able to accommodate.
Employee stated while operating the man lift squeezed finger between motor and rail on lift. This is reportable because of stitches & fractured middle finger.
Cleaning under crusher belt and a stone fell off and hit the employees back.

Employee reached in between the cutter teeth and table at the same time as a co-worker engaged the table. Employee's middle finger was pinched.
Employee was removing a pin on the rear axle of a piece of equipment when the pin came out and dropped on his left thumb. He was taken to the hospital where the thumb was surgically repaired and he was released to sedentary duty. He was subsequently placed off work.
An employee was prying worn liners off the feeder wall when the liner above fell on another employee's arm.
Injured employee and two other employees were lifting a 5 foot by 5 foot by 1/4 inch thick piece of steel (approx 275 lb) onto a truck. Injured employee felt something strained in his shoulder. Doctor diagnosed a rotator cuff tear on June 7.
The employee was putting a primer box up in the top of a magazine and the box fell down. He tried to catch the box and felt something pull in his shoulder. The doctor put him on light duty at that time. He continued to have pain and the doctor put him off of work until he could see a neurosurgeon. Date 07/26/2011. The employee was on light duty from 05/23/2011 to 07/26/2011.
EE was a passenger in a golf cart, going out of the mine after his shift, when he struck his head on a roof bolt extending from roof.
Employee was in mantrip when it reached the top of the hill and the drive shaft yoke broke. Mantrip started backwards. As the wheels sped up, it jumped and cut into the rib causing it to stop suddenly.
While standing in a rowboat to weld on a floating conveyor and although the boat was properly secured, it shifted slightly causing welder to lose balance. He twisted his right knee in his effort to recover.
Right Hand caught in drill carrousel.
Unfolding a radial stacking conveyor belt got pinched and snapped causing damage to 1 of the troffing rollers causing roller to fall and strike the EE in the left forearm, breaking on of the bones in his forearm .
One employee was cleaning out under a conveyor at IA with the cat skid steer and the injured employee came in behind him and started to clean conveyor I5 when the driver backed up over the injured employees leg.
Employee was moving a truck jack. His right hand slipped off the handle, catching his hand between the jack and the bumper of a Liebherr truck. Employee received a cut to the right ring finger that required stitches.
Miner was leaning over with his right hand on the ground, checking for ore. A rock rolled onto his right hand (the hand that was on the ground), striking his #5 finger. He was taken to ER Room for x-rays, and it resulted in a fracture to the #5 finger. He was issued no work until 5/23/11, at that time he will be doing ""Light Duty"" as we can accommodate his restrictions.
He was operating the shearer when a piece of rock came off the headgate drum and struck him on the right hand. *We dispute the allegations in Citation # 8049157 and submit this report solely to terminate the citation.
EE and another employee was carrying a sand prop when ee lost his footing and fell, in the event to catch himself he run his left hand across the flat sharp edge of the prop he was carrying. EE suffered a laceration to his left hand in the thumb area requiring 5 stitches.
Employee was loading a barge and when he was walking he slipped and hit his back when he fell.
Employee fell 3 feet from working platform and fractured 4 ribs on a piece of iron. Employee had a well lit area but improper footwear and was in too big of a hurry.
employees hand slipped off the hand rail of his truck while he was climbing into it, it was raining and the handle was slippery.
Employee stopped mantrip and put on park brake to change drivers. His foot got caught on something and he fell off of the mantrip. Mantrip was NOT moving.
As employee was moving the rub rail on a 14CM15 Continuous Miner he felt a pulling sensation in the lower left abdominal area. Employee was diagnosed at Med Express with a hernia on 5-17-11. Employee returned to work on restricted duty from 5-18 to 5-24. The hernia was repaired on 7-27-11 which the Safety Department was not notified of his surgery until 8-3-11.
Employee was pulling rock off the head of the miner and caught his right hand pinky finger between the rock and the pan on the miner.
Injured employee along with another employee were installing a chute at the secondary crusher, they had installed a 20' x 30' of 3/8th plated steel in the chute. Once the plate was in place the injured employee had to make a minor adjustment and was repositioning the plate when he felt a sharp pain in his left arm. Resulting in a strain to the bicep muscle
He was standing on the third rung of a ladder. He fell backwards and landed on his back causing pain. *We dispute the allegations set forth in Citation #8051189 and submits this report solely to terminate the citation.
EE said he bent over to go under the sill leg at the S-60 when he heard a ""pop"" in his back, he sat down, said he'd be alright, minutes later he said he was going home to rest his back, left the Quarry at 10:47am. He called the Quarry at 11:20 in extreme pain he was at the C.I.M.C., a nurse took him into the Hospital, failed to file report before leaving on his own.
Employee was lifting a 5 gallon container approximately 18 inches to a shelf. The employee rolled his ankle causing him to stumble and thereby causing a suspected muscle strain to abdomen. Employee did not feel injury was serious. Employee sent to physician and it was diagnosed as a hernia.
Employee was bolting when a rock fell & employee leaned one way, and rock rolled striking him in the knee, causing it to go the other way, a possible strain/sprain. He reported it & went to medics on site. On 5/20/11, he went to doctor who issued restrictions in which we could accommodate.
EE was bolting in the #2 cross cut from Entry 4-5 in the 2 Rt. Room off of 2 Rt. Submain when a rock fell aprox. 18"" X 24"" X 2"" thick. As he turned away he twisted his right ankle and was struck by the rock. Accident was not reported to management until 5/17/11.
Clearing lodged material from tail gate on dump box end gate slipped smashing 2nd finger of right hand.
Employee pulled some electrical cable by hand to move it from one area to another and felt numbness and tingling in his left elbow and fingers.
While getting off the #16 drill, when he was going down the boarding steps, the bracket came loose from the catwalk, he caught himself with his right arm. During this process he twisted his left knee resulting in a Knee Strain.
Employee was connecting a chain from the stacking conveyor to the front end loader to reposition stacking conveyor. Loader began backing, chain tightened, and employee did not release chain. Employee strained right shoulder.
The employee started to lift a metal screen up a 5 foot space when he heard his right elbow pop. He could no longer use his right arm. The result was a contusion of the right elbow.
EE stated that he was replacing the CLA on the conveyor pot of the miner. As the mechanic was raising the pan on the miner EE said that he stumbled and fell onto the pan. His left hand was pinned between the stop and the pan resulting in broken bones in the hand. EE was treated at the University of Tennessee Hospital and had pins and plates put in his hand.
Employee was splicing a belt when he reached down with his hand to check the rivets when he was struck by a sledgehammer causing fractures to his index and middle finger.
Moving Highwall Miner Cable and got tangled in cable. Fell & strained back.
Craftsman was working to replace a tire on a 24H motor grader when his fingers entered the framework of a hydraulic push tool resulting in a crushing injury to his left hand.
Walking out to service truck in ice and snow, slipped and fell, broke left leg and ankle.
Employee was attempting to move belt, he was pulling using channel locks when he slipped off and fell back. He had pain in his lower right back.
He was walking on the portal bottom when he felt pain is his left knee. The operator disputes the allegations set forth in Citation #8051190 and submits this report solely to terminate the citation.

Contractor driver dumping and had stated she was uneasy in area of dumping then while lowering bed of trailer, the truck turned over resulting in damage to truck/trailer and driver experienced pain to shoulder and neck resulting in work restrictions.
While a mechanic was hammering a track pin on Shovel 3, a fragment of metal dislodged from the pin and embedded into the upper leg of an employee who was standing approximately 3-4' away.
Lifting acetylene bottle and felt pain in groin & low back. Continued to work until 6/21/11.
Installing bottom rollers on D10N. Used bottle jack on corner of major bogie to lift. Jack slipped and pinned left hand between roller and the track
The employee was checking the amount of supplies on the truss bolting machine. He was positioned on the operator side of the machine with his left foot near the front tire. As the bolter was trammed forward, the employee's foot was caught-under the machine tire resulting in a fracture to the employee's foot.
He was stepping off a supply car when he slipped and landed on his back. The operator disputes the allegations set forth in citation # 8051138 and submits this report solely to terminate the citation.
EE was replacing a bolt in the cable stand off, his ratchet broke causing his elbow to strike the miner. EE did not seek medical treatment until 7/28/11 when it was determined he had a slight fracture to the left elbow. He began missing work at that time. 8/8/11 when he returned on restricted duty, on 8/15/11 he was given a full release.
Was putting a curtain board up & hit elbow on t-bar causing laceration. Currently off work for bone infection unsure if related.
Employee stated he was changing screens. He had failed to fasten the chain at the point of egress losing his balance at said point and fell to the ground.
Employee reached into a hole to remove a blockage, in the course of pulling the rock out, felt his wrist pop.
Pulling the power cable to the electric dredge when the cable was going over a chain that was hooked to the railing the rope slid off causing the railing to come loose and strike the ee knocking him down. He grazed his head on the edge of the dredge air conditioner. He also injured his left knee.
A 9"" x 7"" x 77""long cribbing block was standing on end, ee went to remove block when it tipped over pulling him down with it. His thumb got smashed between the block and concrete floor also with his body weight.

The #7 Mine uses bed ash to make roads with, it is similar to concrete. EE got bed ash on his pants and later got wet, he started operating a scoop. A short while after operating the scoop his legs broke out in a rash. He later went the Dr and the results were second degree burns.
Employee dropped a steel I beam on his foot while going general clean up around mine office.
While pulling at a hose, EE stated that he had hurt his back. The preliminary diagnosis was a muscle strain.
EE was swinging sledge hammer, trying to break nut on cone head loose, felt ""R"" knee pop. EE went in and had MRI taken, resulting in surgery on Monday, May 23rd. EE did not lose any time or work restriction until May 23rd.
EE our Lab technician was removing sampling trays from the Gilson shaker. She did not have a firm grip on the tray and it came back towards the top of her right knee. The tray hit this area resulting in swelling and pain. She tried ice it but it was still swollen and it was hard to walk on.
Employee was installing roof bolt when pan fell out onto controls causing the bolter arm to swing over and striking the employee. Employee was put on MLOA on May 22,2011.
Employee was lifting the landing plate with the driller helpers when he felt pain in his abdomen. It progressed to pain in his groin. He continued to work and saw a doctor on June 2, 2011. He originally received a full release but was later re-evaluated and placed on restricted duty.
When getting ready to operate haul truck 419 employee tripped on loose deck panel, fell to her knees. Most of the weight went to her right knee and right hand.
Employee was pulling a 3"" hose to hook up to pump. When pulling hose he felt a strain to lower back and left arm. Employee did not seek medical attention until 6/29/11.
Employee was operating long reach fork-lift. When exiting the fork-lift the employee stepped down to the ground with his left foot. When the employee stepped down he indicated that he stepped on a piece of limestone and experienced pain in his left knee. Medical evaluation of the employee indicates that the employee suffers from Degenerative Joint Disease in the left knee.
Employee was checking offside of CM to check bottom, rock fell from between bolts striking CM and then landing on employee. The rock struck employee on shoulder and leg area. Employee was taken outside and transported to Greenbrier Med. Center and later transferred to CAMC. Treated for contusions and released next morning
EE was bending a roof bolt to install in a low spot along the rib, when he felt a sharp pain in his right elbow.
As box was reaching max angle ee felt truck began to lean to drivers side. Immediately began dropping the box. Truck laid over on its driver side door when the box was down to about half way. Hit knee on the door. Went to ER to be checked & returned to work w/o restriction. EE return to DR on 5/31/11 with knee pain. Crutches & immobilizer restriction.
EE squatted down to lift 3 cardox tubes. He placed them on his shoulder. When EE stood up he felt a pain in groin area.
Working with another EE on pump. EE placed a bar to hold sheave and the other EE asked him to move the bar so he could check the threads on the shaft. When the other EE tried to screw on the impeller the EE had not replaced the bar and tried to hold the sheave by the belts. EE's fingers were caught between the drive belt and sheave.
Was changing a belt roller. Had a burning sensation in abdomen area.
Miner was repairing a Hercules roller mill plow and was removing a bolt from one of the plows. when he struck the bolt with a hammer cement shot out of the hole and into his face and left eye which was scratched.
While removing the lock ring on a haul truck tire rim on #4 tire the pry bar slipped smashing his left hand ring finger against the wheel motor.

Rock bridged in chute from 6x20 to Belt #3. Must clear from chute on bottom side. When rock let loose, EE was unable to get his hand out before rock smashed fingers.
Employee was removing a door when he cut a guy wire the hydraulic cylinder came down on his thumb.
while assisting to remove a screen EE felt a pop in his neck.
Employee was trying to attach a tractor cable hook to a gun sled. While twisting the hook to line it up with the pad eye on the gun sled the hook slipped and twisted back pinching employee's right pinky finger between the hook and the gun sled resulting in laceration to finger.
Worker received a shoulder strain while pushing down on a pry-bar during repairs on a front end loader. He requested medical care the morning following the incident.
EE was getting off haul truck when his ankle rolled on the ground. EE was taken into the clinic to be checked. EE was given an ankle brace and placed on restrictions. EE returned to work same day on a restricted duty status.
Employee was removing impact slides from conveyor belt/chute. His left middle finger was pinched between the slides' handle and a 5"" channel beam. He was given 3 stitches to close the wound and put on restricted duty for 2 days.
Employee was struck by a small coal rib which caused a sprain to his right ankle.
Employee used drive belt as a hand-hold. As employee pulled on the belt it moved; causing his hand to be caught between the belt and pulley. Equipment was NOT energized.
Miner was rebuilding a hercules mill roll for installation and was using a 3# hammer to drive a bolt into place and after several minutes of swinging the hammer he felt a pop in his lower back.
A piece of 11' drill steel fell out of a holder on a drill unit, the employee reached to catch the piece of steel and caught his finger between the drill steel and the retractor wheels.
He was trying to pull auto rodder, working in awkward position using 24 inch and 18 inch pipe wrench when he felt pain in his lower back. He received prescription medication and restrictions on 5/25/11.
Worker received a back sprain while operating a D9L bull-dozer to back-fill a surface mine pit.

The employee was carrying a core barrel, stepped on an uneven surface and twisted his right ankle resulting in a sprain. He worked four days of restricted duty.
Employee was removing bolts from discharge pipe. He was pulling upwards on a bolt with a 24"" ratchet. While pulling up he felt a ""small pain"" in his right shoulder. Originally treated conservatively as a minor first aid, became reportable on 8/12/2011 when physician placed employee on restricted duty.
While walking up stairs carrying a shaker screen he felt a pop in his right big toe. Worker notified supervisor of the situation and an injury/incident report was filed. The employee later went to the doctors office and was given a full release to return to work. On 06/01/2011 employee was referred back to the doctor and was placed on restricted duty.
Employee was using a sledgehammer to dislodge hydrate from the K1 hydrate feed bin, and strained his left shoulder.
While changing a turnover roller the bearing housing turned, catching the right thumb between the housing and the housing base, crushing the tip of the thumb.
Laceration to 2nd finger left hand received when employee was handling a filter while working on the shovel.
Employee was servicing a mobile crane on the apron and felt pain in his back while lying underneath the rear differentials.
Employee was climbing up a 7 foot vertical ladder and strained his lower left back. Employee is on light duty.
