# 3 hoist went down with a PLC issue at 5:50 am. MSHA was called at 6:10 to alert them. Evacuation was started. The evacuation was called off when the issue was resolved at 7:00 am. MSHA was notified that hoist was running at 8:00 am.

An unintentional roof fall occurred just outby 6 x-cut in 4 south mains. This fall occurred inby the 2 east section four way, in the old I track entry. The fall measured approx. 60 feet long by 16 ft wide by 8 foot above anchorage. No injuries or equipment damage as a result of this incident.
An unintentional roof fall occurred on the Main West 2 Right Return between Entries 10 and 11, Crosscut 82. The roof fall measured approximately 55' in length, 18' in width, and 7' in thickness.
A roof fall 15'x15' and 0' to 6' was discovered in the intake affecting travel
An unintentional roof fall occurred in the #3 entry of 5 H. This fall occurred at 6 x-cut. The fall measured approx. 18ft long by 16ft wide by 10 ft high. There were no injury or equipment damage due to this incident.
A wire termination for the #2 gear was damaged. Miners were evacuated at 3:14 a.m. and on the surface at 3:24 a.m. An electrician was called out to repair the damaged wire. The hoist was tested and miner returned underground at 4:25 a.m.
A roof fall occurred at Block#137 Track entry/#6 entry in a intersection plus approx. 15' each way past the rib line of cross-cut. Roof was slate-fell approx. 8' H. X 18"" W. approx. 45' to 50' in L. Area had been bolted with 6' torque tension bolts. Area was developed approx. 9/1/2008.
The fall was at survey station #317 in the #7 entry, 1800 ft from the face. The fall was 5 ft in height, 20 ft in width and 60 ft in length. The fall will not be cleaned up. The entrance to the fall is dangered off and supported as per roof control plan.
A fire was discovered at the solid waste dump at 2:00 AM. The dump is located in a remote part of the mine so at no time were mine personnel or mine facilities threatened. The cause of the fire has yet to be determined. The fire was extinguished at 2:45 AM.
An unplanned roof fall has occurred, above permanent anchorage, on the 11 north miner section. This fall exists in the #4 belt entry, one crosscut outby spad #566 and measures approximately 50'X 20"" X 10' high.

Roof fall at x-cut 87 in #3 Entry of the Tailgate Unit. Above the anchorage. The fall was at the face. 002 Unit.
The fall was discovered on the #5 Main Belt (#35 - #36 block). The fall was located 1050' outby the feeder. The area was bolted with 8' Super Twist bolts, 16' CC cable bolts with T5 channel. There were also cribs and 4 hienzman beams. No travel ways, escape ways, people, or equipment were affected by the fall.
Discovered roof fall at approx. 4:05p.m. on the A West Panel, 200' inby survey station 6254 in the #2 neutral entry. The Roof Fall was at or above anchorage zone of the roof bolts installed in that area and the fall did not impair ventilation nor impede passage or travel of personnel. The fall meas. 30'L. X 10'W. X 8'H. Currently there are no plans of cleaning up the fall.
Roof fall was discovered inby Spad 30875 in the #3 entry by Mine Examiner during Preshift Exam. The roof fall measured 20 foot wide, 20 foot long, and 10 foot wide. No injuries were reported as a result of this roof fall.
Roof fall was discovered adjacent to Spad 30417 by Mine Examiner while traveling return air course. The roof fall measured 20 foot wide, 160 foot long, and 10 foot wide. No injuries were reported as a result of this roof fall.
The kiln ID fan shut down. While the kiln was down the coal pipe boot caught on fire 4:15 am till 4:45 am and was put out 30-35 min. later. MSHA was called during this time while our employees were putting out the fire.
A rock fall occurred on the #1 South East Mains in the #7 entry at crosscut #27. The fall measured approximately 20' wide x 80' long x 8'-10' thick.
Brake sensors were out of adjustment causing a brake failure and the hoist to trip out. There was no damage but mine was evacuated until sensors were properly adjusted and hoist was operational.
Elevator in shaft #1 was taken out of service by hoist operator due to brake problems. He notified EE and he began to make Phone calls to EE who sent field techs out and evaluated the situation. He also gave instruction to evacuate men and called the State and Fed emergency hot line #s at 9:05 AM.
A roof fall was discovered in the 6B off 5 North miner section. The fall was in a 3 way intersection at survey station #125 + 86 in the #2 entry. The fall was 16' high x 20' wide x 24' long.
Roof fall was in the 3rd Northwest Butt Room section in #23 room 45' inby spad # 2082 The fall was approx. 55' L. in #23 room and fell through X.C. 30'L. towards #24 room 18' W. unknown H. Area was bolted with 6' bolts and 8' cable with some 4' super twist outby fall area these were vertical joints and slips.
Fire destroyed maintenance garage from unknown cause.
A roof fall was discovered at crosscut 10-sc. Dimensions measure 10 feet high by 40 feet long and is rib to rib.
At approx. 1:20 am the elevator at the Hart Street Portal became inoperable. Upon investigation it is believed that the newly installed guttering system to help reduce the water in the slope came in contact with the trailing cable shorting it out.
Foreman was doing his weekly return exam and found a roof fall. 160 X 20 X 5 in the 4th panel of 001 section. This panel was stopped 3/10/08. This is about 3200 #7 from 001 section. The fall was cribbed and timbered and dangered off. MSHA was with foreman at this time.
Crews were evacuated through the Hicks Shaft due to a contactor being down on the hoist. The problem was found and repaired at 8:15 am. MSHA was contacted.

The Man hoist was taken out of service to perform unscheduled repairs to a cracked brake shoe. All miners were evacuated out of the mine prior to taking the hoist out of service no emergency evacuation needed, MSHA was contacted as a courtesy, to report the repair, the OEM was brought in to make the repairs.
The elevator at the camp creek portal was down for more than 30 minute de to a door switch being bad. The elevator was repaired and put back in service. Person could travel via the slope by slope car or walk while the elevator was down.
Concentrator Step III - elevator stalled for 2 hours 45 minutes - called MSHA @ 8:22 a.m.
An unintentional fall occurred at the 7+00' crosscut in the 2nd west between #1 and #2 entrys. The fall was14' wide by 10' long by 7' high. Fall area was timbered off.
Fire was started beside railroad track in a remote area of the mine. The fire was extinguished by the Mt.Pleasant fire department at approximately 7:35 p.m. No damage was done to any equipment nor injuries to personnel fighting the fire. No miners were involved in fire fighting efforts. Cause of fire was not determined.
On or about 6/21/2011, an unplanned continuation of a roof fall occurred. The fall was located at cross cut #197, #7 entry, in the Main West entries. The area was bolted with 6' point anchor roof bolts.
A roof fall occurred between 1 and 2 entry at crosscut #5. The fall was from 6 to 8 feet high. The fall was approximately 30 feet in length. The roof had been bolted with six foot fully grouted roof bolts.
An unplanned roof fall above the anchorage zone occurred outby the #1 seal of the No.2 main seals, one break inby spad #1132. The fall measured 42ft long X 20ft wide X 10ft height. No injuries, entrapment or equipment involved. Ventilation was involved to the sea.
A power outage caused an evacuation of the Coy Mine, the cause of the outage was a thunder storm. MSHA was notified and the mine was inspected prior to returning to work.
A fall of roof occurred on the #1 unit in the last open crosscut in the 4th S.E. panel at crosscut #27 in the #6 entry. Personnel nor was there any equipment affected. The fall measured 19' by 19' by 8' ft high.
Heavy rains caused water to go down the shaft and the water damaged some electrical components of the elevator.
Unplanned roof fall occurred break #13-#14 in #2 entry of 3A belt at spad #1003. Area was developed September 2008. Fall was approximately 50 ft. length X 19 ft. width X 13 - 14 ft. high. No injuries or entrapment involved. No ventilation, escapeway or equipment involved.
A roof fall was discovered by mine management. The fall was reported because it impeded travel. The mine is currently in a non producing status.
An unintentional roof fall has occurred on the 3B-submain. The fall was located in the #4 entry of the #0 xc. The fall was approximately 18 feet in length and 18 feet in width and approximately 10.5 to 12.5 feet in thickness.
Electrical Contact burnt out making the hoist inoperable, the mine was evacuated and MSHA was contacted. The hoist was down for a total of 27 hours. Repairs were made and the mine was examined prior to reentering the mine.
A rain storm in excess of 5.74 inches of rain occurred at the mine causing an inundation of water into the mine resulting in 3 3rd shift employees being trapped underground. The employees were rescued at 8:20 pm.
The #1 elevator was out of service for a period of time exceeding 30 minutes. No persons were on the elevator at the time. The vents in the elevator controls were cleaned and additional cooling was provided for the controller. The car was tested and placed back in service @ 4:10 PM. All necessary government agencies were notified according to law.
The Somerset elevator failed to operate when called by an underground employee. The elevator contractor was called to the mine to repair. There was no damage to the elevator, the repair consisted of a door adjustment. There were no persons injured, or entrapped due to this event.
The K3 elevator stopped between the second and third floor and two contractor laborers were inside; one laborer from Labor Finders and the other from Southern Refractories. Executive Elevator was called to reset the elevator.
Employee started transferring carbon from the storage tank into the silo when an ignition occurred which blew the top safety hatch open. This was a rather large ignition that resulted in fire shooting out the top of the tank and light up the night sky. When the ignition occurred the employee was standing between the storage tank and silo.
This area of the mine is on return air. This area has been danger off and timbered off for a while.
An unplanned roof fall occurred on 1 North belt 22 x-cuts from the tailpieces. This area was mined around Feb 1980. The fall was about 5 foot high, 40 foot long, and 19 to 20 foot wide. The area was bolted with 8 foot point anchor. The fall was cleaned up and rebolted and it was cleaned up by the clean up plan.
B-Section was pillaring in the #6 entry when the MRS became stuck and required assistance to become free.
Operator backed the 8000 gallon water tuck 320 yards down a 10% road and when he attempted to stop, the brakes failed. He steered the truck into the highwall to avoid hitting a 100 ton haul truck or 992 loader, which caused the loaded water truck to roll over. The driver did not sustain any injuries. Damage occurred to the cab of the water truck.
The service elevator went down because the relay contactor would not allow the breaks to release on the elevator. Changed out a input card and main contactor and the elevator was put back in service at 8:20 am.
Unplanned roof fall occurred break #13-#14 in #4 entry of 3A belt at spad #984. Area was developed September 2008. Fall was approximately 50 ft. length X 18 ft. width X 14 ft. high. No injuries or entrapment involved. No ventilation or equipment involved. Secondary escapeway was involved.
# 1 Hoist kicked out abruptly with multiple electrical faults. Suspected electrical issue. Repaired by fixing short in east transformer.
An unintentional fall of the roof was discovered on the 7 left panel at #71CX in the #1 entry. It was 18 ft x 100 ft x 7 ft. There were no equipment or persons effected by this fall. It will be cleaned up and the plan has been submitted to MSHA and approved.
EE WAS TRAVELING IN RETURN WHEN HE FOUND A ROOF FALL, ONE BREAK INBY SPAD NO 1852 ON NO 2 SW MAINS.THE FALL IS 25 FT LONG BY 20 FT WIDE BY 7 FT HI.THE FALL IS 10,000 FT FROM THE FACE.
50L x 20W x 5H rock fall in #4 entry inby 0536 (Spad) on 1 south belt.
Foreman was doing his weekly exam out by returns and found a roof fall 150' X 20' X5' long. He dangered off the area and timbered it off. This area is an old panel, 2800' from the working section.
During hoisting, latch arm on N skip allowed skip gate to open when skip going down, and damaged gate. Hoisting stopped; mine evacuated; NO injuries or other problems. Day shift crews sent home.
The eye on the jack that operates the brakes broke causing the cage to stop on the bottom of the shaft.
An unplanned roof fall occurred in the 1st West Mains in #1 entry. The fall measured 15'wide x 16'long x6.5'high, partially blocking the travelway. The fall was discovered by the examiner during his weekly inspection.
During hoisting, a switch malfunction allowed skip gate to get out of sequence and catch on crossmember support in headframe and caused slack rope that stopped hoist. After investigation, slack rope taken up, test run done, and all OK. NO injuries.
An unintentional roof fall occurred on the 1 North 2 Main West 1 Entry 8 Crosscut 93. The roof fall measured approximately in 45' length, 18' in width, and 12' in thickness.
An unplanned roof fall occurred at survey spad 3106 #3 entry inby dumping point. The roof fall was approx. 40'x18'x7'. The initial 103(j) Order was modified to a 103(k) Order. Both Federal and State agencies approved a clean up plan to rebolt and clean roof fall. On June 17 the 103(k) Order was lifted to resume normal production. No injuries was reported or no equipment involved.
The surface Westinghouse transformer located on the surface at Substation #1 was noticed to be on fire. Upon investigation it was determined that a Yellow belly Marmot had crawled up on the transformer and shorted it out.dept. The damage resulted to limitations to underground operations, total loss of transformer, electric structure and the use of one main ventilation fan.
A D-ii operator noticed smoke under the cab of the D-11 operator working beside him. The operator of the D-11 that was smoking shut the machine down and with the help of the foreman tried to extinguish the smoke after about ten minutes the fire department was called because they could not get the fire out. The pit was evacuated at this time.
Roof fall on the Tailgate Unit at x-cut 87 on MMU-O02 in number 2 entry at the face area.
A roof fall was discovered in the primary escape way #5 entry at crosscut 33 outby 001-0 at 6:50am. The fall was approximately 14 feet high x 19 feet long and 80 feet wide. The fall did not affect persons, nor ventilation. It was cribbed, cleaned up and an arch installed.
An unintentional roof fall occurred on the 5A travelroad at 52 crosscut on the 8th West panel. The fall measured 25 feet x 20 feet x 6 feet high. The fall had been bolted with 6 foot fully grouted roof bolts. The fall did not affect ventilation or cause personal injury. The fall was in the travelway so the secondary escapeway had to be re-routed.
A roof fall was discovered at 8A longwall in the #3 entry. The fall is above the anchorage and measures 21 ft. in length.
Weekly examiner located the fall while completing his exam of the area
A roof fall was discovered in 58 longwall tail gate Entry from 29+85 to 30+01. Dimensions measured 25 feet long by 15.5 feet wide.
A roof fall occurred in the #11 seam in the 2 Main North in the pillar line between crosscut #74 and #75 in the #4 entry. The fall measured approximately 35'x14'x1'-5'. The area was supported with 3' fully grouted roof bolts and an undetected slip was determined to be the cause of the fall.
Rock fall occurred on the 1 East Mains. It fell at the #5 crosscut from the #8 to the #9 entry. Fall measured approximately 20' wide x 110' long x 6'-8' thick.

ROCK FALL FOUND ON SLOPE AT KRONOS MINE AT SPAD # 1050+5 AND NOTIFIED MSHA BY PHONE JUST AFTER
The wana service elevator was damaged while hoisting a scoop cay. The hook broke free arrowing the scoop to fall causing damage to the side wall, floor of elevator and winch.
A roof fall above the anchorage point occurred on the 1 North #2 belt at #40 crosscut in #5 entry. No injuries resulted from the fall. Ventilation was not affected. Damaged equipment included belt structure. Dimensions of the fall were approximately 15.5 feet wide, 16 feet tall, 30 feet long.
On or about 06/09/2011 an unplanned roof fall occurred on the 3rd N.W. Sub Main in #3 entry, cross cut #5. The area was bolted with 8' & 10' point anchor roof bolts. The fall will not be cleaned up. The area was fractured laminated stack rock. The fall was approximately 17 3/4'wide X 18' long X 10' high.
While baling hay a bearing on the baler got hot causing the bale in the baler to catch on fire. When the bale was kicked out of the baler automatically it caught the grass on fire. The baler was not damaged and there were no injuries. The fire was extinguished but burned more than thirty minutes.
The elevator was down for more than 30 minutes due to a coil being down that caused the brakes to lock up. The coil was changed out and repaired and the elevator was put back into service. People could travel via the slope by slope car or walk up the slope while the elevator was down.
# 5 Hoist electrical fault causing hoist to not operate. PLC control enclosure had a temperature of over 120 Degrees. Cabinet was cooled down and hoist was operational at 9:00 PM. Two other hoists were available for U/G miners to enter and exit the mine when the hoist was down.
Material being dropped from the sheave wheel platform of the Jewell Shaft head frame struck a power pole guy wire causing power conductors to make contact and trip out the power to the Jewell Shaft hoists.
Long-hole blast induced failure occurred during the 3rd long hole blast between the 622 and 490 elevations. The fall extended above the height of the access and brought down material in contact with a known fault area.
The elevator in the #6 vertical shaft was inoperable due to failure of a solid state relay in the control circuitry. The elevator was inoperable from 5:20pm until 7:00pm
Roof fall in intersection on the #1 belt conveyor. Slickensided roof conditions in the area. Transition zone between sandstone and shale roof.
The Cedar Creek Elevator was out of service from 8:00 pm until 11:30 pm due to a Field DC Contactor.
At 10:07 am the service elevator went down due to the elevator door switch. The elevator was put back in service at approximately 11:00 am.

An unintentional fall occurred due to adverse conditions above bolt anchorage at the #40 crosscut 28+00', on 2nd West Travel way.
Found an unintentional fall at 1st Left off of Left Horn Branch - Break #6 - #9 Heading - Fall was cribbed and dangered off & supplemental support was added to the affected surrounding area.
The emergency hoist became inoperable during an inspection by MSHA. The braking system would not release. A minor adjustment was made to the brake system and the hoist returned to normal operation at 10:55 am.
The steel braided oil supply line running to the left hand rear turbo charger ruptured. The oil sprayed onto and around the engine compartment area causing the fire.
A roof fall occurred on I-0 Section at approximately 3:15 am in the return between cross cut 32 and 33. The roof fall measured approximately 50'Long by 20'Wide by 6 to 8 feet high.
Employee was traveling 1 south east main section and found rock fall at Break 34-35. Fall is approx. 175ft long, 20ft wide and 6-10ft thick.
The emergency escape capsule at the #9 intake shaft was inoperable due to malfunction with the electric brake. The capsule was dropped down approximately 300 feet when the electric brake locked up and would not release. This report is being submitted subsequent to citation number 7022047 taking capsule out of service.
A roof fall was discovered in #4 entry inby spad 3106 and outby spad 3145 for a distance of approximately 420ft. No injury resulting from fall.
Following a routine maintenance by Schindler Elevator service, 2 employees were briefly entrapped on the mine' s passenger elevator. The elevator was lowered to the bottom where the employees got off. Schindler Elevator service was recalled and found and replaced a defective governor rope slack switch. The elevator was returned to service at approximately 6:15 PM.

Long wall return entry of 16 Left Long wall had a roof failure at 182+16 extending outby for appox. 30 ft.
A supply trip was being hoisted at the Somerset Slope. A pin broke on a drawbar, along with a safety rope which was connected to the trip. This caused the cars to travel back down the slope and de-rail at the bottom. There were no persons injured or equipment damaged as a result of this occurrence. This report is being filed to satisfy the MSHA Inspectors actions.
An unplanned roof fall occurred in intake on the 1st northwest mains in the #5 and 6 entry. 6800' feet from the face.
A roof fall was discovered in the primary escape way 3 crosscuts inby #2 belt drive at spad #1203. The fall was approx. 30' L. X 19' W.and had fallen above permanent roof support. The fall was discovered and reported to MSHA.
Fall was found on 1st left off of south mains, inby spad #954. Fall was 18' long x 18' wide x 4' to 5' thick. Roof support was 6' torque tension bolts.

A non injury roof fall occurred in the #3 entry of the return on panel 4 off North East mains. Additional roof support had been installed in this area prior to the fall occurring.
the 12 east longwall bleeder had a methane inundation which was found by company representatives and msha inspector during normal msha inspection trip.
A mixture of coal and petroleum coke started to spontaneously combust in the # 3 coal bin. This is being reported because the fire lasted more than 30 minutes.
The mine suffered a gas or dust ignition in the # 4 heading, there were no injuries. The continuous miner was removed from the face and the area was re-ventilated.
While mining from the tail to the head, an alleged ignition occurred while checking the head coal thickness at the head shearer drum located near 195 shield on the 18A panel
A roof fall occurred on 6/2/11 at spad # 13 & 30 in the #2 return entry. Fall was 18'W. X 30' L. X 6' H. Did not affect flow of air and will not be cleaned up. No equipment involved.
The Dry Ridge Elevator was unoperational. The elevator stopped in the shaft due to a govenor switch alarm. There were 3 employees on the elevator at the time it stopped. No injury's as a result of this incident
An unintentional stoppage of the man/supply hoist of A Shaft occurred at approximately 8:00 PM causing damage to numerous wooden guides in the shaft.
An unintentional fall of the roof was discovered on the 1 Right Tail section panel in the # 3 entry at spad # 90 cross cut #42. The fall was approximately 18ft. x 18ft. and 7 feet high. This was in the bleeder entry and will not be cleaned up.
An unintentional fall of the roof was discovered on the 8 left section panel in the #3 entry at spad #30 cross cut #13. The fall was approximately 40 feet long and 18 feet wide and 7 feet high. This was the belt entry and secondary escape way. All employees were withdrew safely to a location out by the fall area.
An unintentional roof fall occurred in the No.6 entry at spad No. 2577. The fall measured 20' high, 20' long and 6' high. The fall did not affect the air flow or the travel of men.
K&S Timbert Company employee turned over a logging cutter at the sand plant which caught on fire. Fire was extinguished. Report was made to MSHA within minutes of notification. The fire was not close to dredge or buildings. Investigator discussed with Dolese on phone but was not concerned about MSHA being involved.
Employee was watching the cable on the dolly when the rib rolled off crushing him. Rib measured approximately 82 by 36 by 11 inches.
Victim was cleaning rocks and coal to set timbers along a rib line. The rib and brow fell crushing the miner. The brow measured approximately 100"" long by 32"" thick, by 37"" tall.
Working on a structure 15' high & was safely tied off to structure, when he completed his work, instead of stepping onto platform from beam he was standing on while tied off - he untied and then stepped and fell 8' onto another 5 1/4"" beam with his chest. He walked to the company truck and sat down ambulance arrived, got in and died in route to the hospital.
Employee was working without fall protection and fell to his death.
Employee's foot was pinched in Traverse Drive Unit in Crushing Dept. Further investigation pending
While repairing the dust section hose on the roof bolter, EE accidentally placed his left hand in a pinch point then lifted the head causing the leveling arms to smash his ring finger. He received several stitches to close the wound.REVISED Reports from WC show an amputation of the tip of the ring finger.
EE was clearing material on a belt. He called Control Room (C.R.) to restart the belt. Belt started, but soon shut down. C.R. called the employee but got no response. A 2nd employee found the 1st employee semi-conscious. Employee was airlifted to a local hospital. Employee suffered a right arm amputation at the elbow and numerous fractures and broken bones in his torso.
Employee was lifting a core sample box when he felt a pull/pop to the right inguinal area.
Pulley was slipping and guard was opened and tried to pull slack by hand to get them to stop slipping it worked and took finger with it.
EE was helping to install new guard on a conveyor drive. While in the seated position, the EE felt a pain in his groin area. He stood up and felt a knot in his right groin area. Went to the doctor and had a hernia. The doctor restricted activity for 10 days to no lifting over 10 lbs.
While working on A/C on truck, reached into fan and cut tip of right hand middle finger almost completely through.
Employee performed repetitive prying tasks at the rotary breaker head chute. On this day, employee states he pryed loose a rock in the head chute. Several days later, the employee states he found a lump on his groin area. It was diagnosed as an inguinal hernia. The employee is also a weight-lifter. Employee did not start missing time until June 22, 2011.
Pulling on heavy dragline chain to get it in place. Employee did not seek medical advice until approximately a week later and then was diagnosed with a hernia. After a follow up consultation with another doctor on 7/1/2011, employee in the near future is looking to have hernia surgery.
Miner was cleaning out a conex and was pushing and pulling a cylinder aprox 200lbs, Miner has had a hernia before and failed to use S.L.A.M and correct lifting techniques.
EE attempted to dislodge and remove a rock from a slicer screw by entering the housing from the top. As the screw continued to rotate toward his hand, he pulled it back but was not quick enough to avoid having his left index finger caught between the blade and the rock. The finger had 3/4 of an inch amputated. No lockout was in effect at the time.
The employee was traveling down a hill in a shuttle car on West Main Right section when he hit the rib with the shuttle. A rock measuring approximately 20"" by 16"" by 51"" long came in the cab and hit the employee.
Employee was carrying oil cans from hoist car to the roof bolter. All of a sudden his knee started hurting.
EE and co-worker were handling buckets of plaster across a shuttle car cable. He slipped causing his right elbow to bend in a backwards direction when it hit the side of his leg. He notified company of injury at 6:25 AM on 7/5/11.

Employee was using a pry bar to dismantle and repair a light duty vehicle drive shaft when he sustained right shoulder pain. Was reported as a minor strain and employee would monitor improvement. Smoky Canyon received notification 12/12/11 from workmans comp that this injury was work related after an investigation was completed.
WHILE EMPLOYEE WAS PREPPING HIS TRUCK FOR SERVICE, ANOTHER EMPLOYEE WHO HAD A CONFLICT WITH HIM, APPROACHED AND STRUCK HIM IN THE HEAD WITH HIS LUNCH BOX CAUSING A SEVERE LACERATION ON THE RIGHT SIDE OF HIS HEAD.
Person involved in accident was cleaning a conveyor walkway when a rock fell and hit his right hand annular finger.
Employee mis-stepped over a 4"" piece of pipe to turn off water valve catching his heel and causing knee pain.

Employee was working with 7 other employees in the Dump 1 Feeder Room to clean up a large coal spill resulting from a malfunction of the # 3 Feeder Belt. In the process of shoveling the spilled coal onto the belt to clean up the area, the employee sprained his ankle by stepping backwards onto a lump of coal. This resulted in a lost time accident.

Ee was bolting. A piece of drill steel got loose and hit him in the mouth, knocking out three teeth. Other than needing a partial denture, Ee is in good shape. The dentist allowed him one day off.

Employee was changing tie plates when his finger got pinched between the railroad tie and rail.
Employee stated he tripped while walking along a belt conveyor and as he reached out to catch himself, his left arm was caught between the belt and a roller.
Employee was operating an open cab road grader on a ramp, employee hit his head on a pipe, causing some soreness in his lower back. He went to medics, and was transported down for further evaluation. Doctor issued a few days of rest.

Employee was standing looking at a fuel tank that was sitting on the ground. He turned to walk away when he then felt a sharp pain in his knee. No work activity was involved.
While moving rock from stope to crusher staging area a rock fell out of the bucket. On returning he did not see the rock and ran over it causing him to loose control and hit the rib with his bucket. The jarring motion caused his leg to hit the wall of the cab.
Employee was installing a tensioner cylinder in tailgate using a crane. He pushed on cylinder to line pinhole up and felt something in shoulder snap.
Employee was utilizing a 6' ladder leaned against the drivers side frame to access 2 battery solenoid boxes. When dismounting the ladder he stepped off the first rung in a backward pivoting motion. His right foot was at an angle & rolled upward as he stepped to the ground. The accident resulted in an unspecified ankle sprain.
EE was unloading portable rock duster off of supply car and he lost his balance the tongue of the duster came down and mashed his right hand ring finger and right hand middle finger.
Employee was drilling starter hole and lost his drill bit in the hole. Employee put a roof bolt in the hole & spun it to push the bit up in the hole. While in this process a burr on the roof bolt caught the employee's glove wrapping his hand around the roof bolt.
EE was walking around the head of the continuous miner when he hit his hard hat on a torque tension roof bolt, thus knocking his hard hat off of his head. He replaced the hard hat on his head and went back to work. He continued to work up until his doctor took him off work on 07/29/2011.
Employee was operating truck when he hit some potholes and began complaining with his back and numbness in his legs and feet.

EE was riding in on mantrip, when mantrip ran over rock in haulway,EE stated he hit his head on canopy of mantrip and came down hard on tailbone.
Employee was cleaning 5 entry with a scoop. As he was cleaning the right rib piece of line curtain fell. He went to re-hang the line curtain a piece of rock fell out of the rib pinned right foot. The rock was approx 72x32x16. He was transported to CAMC and was diagnosed with 6 fractures in his right foot. He was given instructions to follow up with an orthopedic surgeon.
Employee fell off 4' retaining wall while cleaning conveyor chute and suffered a lumbar strain/ back contusion
Try to lift 4 inch water with water still in it. EE either strain himself or rupture his self.
Injured was climbing off of his coal hauler and hit his right leg on the steering cylinder. Injured was seen later in the week by an orthopedic and a tibial platueau fracture was discovered.
2 emps. were taking down emulsion line after power move. Piece of line burst, spraying emulsion into one of the emps. face. This caused the employee to inadvertently hit a hydraulic lever on the piece of equipment they were using to work off of causing the injured employee to strike his head on the mine roof.
EE was climbing down a ladder when he lost his footing on the ladder. He caught himself on the ladder rung, but felt pain in his right shoulder. He reported the injury, but thought the pain would go away with time. He sought medical care on 7-15-2011 and had an X-ray that was negative. He was on full duty during the whole time. He did have a medical procedure on 10-07-2011.
Employee was assisting in closing the air lock door when the air pressure pulled down the air lock door pinning the employee against the scoop causing a cracked tail bone.
EE was attempting to clean up area between brattice and 1 set of air lock doors to install 2nd set of air lock doors & to position scoop between air lock door and brattice. EE was maneuvering the scoop, the bucket dumped the brattice, the brattice was pulled down. EE and 2 others were closing the door when the air pressure pulled down the door pinning 2 EE's against the scoop.
While tramming the shuttle car away from the feeder the car dropped into an offset in the haul road jolting the employee causing pain in the employees back and neck. The employee finished his shift but was unable to report for his next scheduled shift.
EE was assisting B & G service technician with replacement of impeller & housing on #5 underflow booster pump. EE was standing 4' from service truck when lightning struck the CB antenna on truck knocking EE to ground.

Employee was changing a suspension spring on a vibrator. While prying it away from the chute with a slate bar, the bar slipped allowing employee to fall backwards onto another vibrator. This caused an injury to the employee's knee.

A bulldozer operator on a D10R Caterpillar dozer went over an approximately four foot tall steeply sloped bank. As the machine proceeded over this bank, the front end cantilevered and dropped suddenly and struck the ground, jarring the operator.
While operating a 777D employee ran upon the berm causing the rock truck to turn over on the right side.
Employee was pulling a dolly with a hydra helper up the stairs to switch pumps. He complained of pain in his lower abdomen. He was evaluated by a physician and diagnosed with a hernia-loss time started on 6/30/11.

EE was working on bolter at Morton Salt. he says as he was positing the work basket I mistakenly operate the lever to close the lower centralizer. My left index finger was caught in the jaws. Crushing my left index finger, resulting in broken bones (index finger) and 11 stitches.
Co-worker was cutting bolt crusher return belt on side wiper. Employee was holding torch hose when he got burn right hand. No gloves on.
Running shakers and sand blew into right eye. Went to doctor and then saw specialist. Returned to work after 1 day away.
Shovel operator was getting off the 16 shovel on the manlift and as she got close to the ground to step off she accidentally may have bumped the switch causing the lift to move in which she fell to the ground injuring her left ankle.
Employee was in the process of shoveling belt when the shovel came in contact with the belt causing employee's left wrist to twist causing pain in his left arm & shoulder. Employee continued to work, but complained of pain in his left wrist, an MRI was performed, he was seen by a specialist on 10/17/2011, at which time he was removed from work with a tear of the fibro cartilage.

On 9/7/2011 the doctor deemed EE's condition as work related. The doctor attributed the condition to vibrations from operating a dozer.

Employee was attempting to open an escape way door. The door was difficult to open due to the resistance created by the ventilation. When the employee tried to open the door, he felt pain in his back. Date of initial injury was 06/22/2011. Employee continued to work until 07/13/2011. Employee decided to seek care on 07/13/2011 from his personal physician.

Employee injured jumping from a parked manbus to avoid being struck by another moving manbus, subsequently striking knee against parked manbus.
Truck #2 while backing up to the dump area, the trucks braking systems failed and the truck began rolling foward. The truck traveled down the road approx. 50 yard and the driver jumped out, the truck cont. down the road about 40 yard hit the berm and then turned onto its right side.
Employee was opening an equipment door when it fell causing him to injury his back. The hinges had broken off the door.
EE was pulling a wall panel form loose from the bottom when it broke loose the top slide down there rib pushing the EE back causing him to fall.

Employee stated: He had been cutting bottoms for a new belt drive. He was beside the miner when a piece of coal rib sloughed off. He had cut 2' of bottoms but had not undercut the coal. The piece was 4""-6"" thick X 2' tall X 5' long. The toe of his boot was under the apron when he jumped and twisted to avoid the falling coal. He broke the navicular bone in his right foot.
Employee was hooking two ends on air line pushing and strained his groin
Employee stated that while exiting the scoop he got his left foot hung between the breaker lever and the seat. The employee stated this caused him to lose his balance and twist his left knee. Possible pulled muscle.
The employee (Plant Operator) was carrying out routine plant walk around when he slipped and fell against sand classifier.
While carrying wooden planks down a set of stairs, an employee lost her footing and fell on the stairs. As a result, the employee felt pain in the lower portion of her back. She continued to work until 6/30. On 7/1 the employee underwent an MRI. 7/6 the employee was directed to stay off work until further tests can be carried out which are scheduled on Aug 6.
EE stated that while being loaded, the loader man dropped a rock into the bed of his rock truck, causing back pain and tingling in his legs.

IW saw fuel leaking under truck, ran under the truck to stop leak and hit his head on the ladder, jamming his neck and back and falling to his knees. He was seen at the ER, diagnosed with concussion and neck/back strain. Released to restricted duty, and then placed off work due to pain.

Victim was standing on the 2nd rung of a stepladder when he lost his balance and fell to mine floor. Taken to hospital where he was treated for a bruised hip or hip pointer. Still off work to this date.

Prying on crusher with bar - slipped off hit him in head knocked him out for a few minutes - needed 4 stitches.
EE was setting a base for a link and lock crib. A rib roll occurred and struck his left foot. The doctor put him on light duty for a few days. Vacation started a couple days later and he did not return. I have made several attempts to contact him and have not got a response from him.
At the end of the shift the employee was coming down from the haul truck stairs when she lost her footing. This resulted in her sliding down the stairs injuring her right knee (laceration) also her low back and neck resulting in low back strain and neck strain.

The miner was giving maintenance to a mobile equip. While holding with his two hands a wrench, he applied force in an upward direction. When his elbows were at his shoulder level he made an extra effort to loosen the part. As a consequence of the strength applied, he reports to have felt a sudden pain in his neck and back muscles.
Operating double boom roof bolter and a piece of rock 6"" x 8"" x 2"" fell and struck his right foot causing a severe contusion.

While raising a lid on the battery of a rock duster the employee was attempting to put the bar in place that holds the lid up for charging, when the lid fell pinching the index finger of his left hand between the battery and the lid.

Employee and another man were carrying piece of stone tread. He was walking backwards, tripped over another piece of stone, fell & twisted right knee.
Employee was loading a rock truck with a 993 loader when the bucket of the loader dropped hard on a rock and jarred his middle back.
Employee was working on the jaw crusher. Employee lifted a 30-pound take-up bearing and stepped back and had a sudden pain in his middle back causing him to double over. Employee received a strain/possible muscle spasm to his middle back. Employee has lost time and restricted time. He has not returned to full duty at this time.
Employee on top of CM cleaning off rock when he slipped, caught himself with arm against rib.
Employee rolled a rock off haul road to remove from travel area, strained a muscle, did not seek treatment at that time. That night he went to the dr. for pain & was released with instructions to use ice and Tylenol as needed. He went to the dr. night of 6/21, referred to Des Moines for further treatment, diagnosed torn bicep muscle 6/22 first missed day Scheduled for surgery 6/28
EE was pulling stones out mill, lot of twisting and turning.
Employee was attempting to place a lid on a container barrel. The employee was bending at the waist while using a twisting motion with his arms and hands. No pain was immediately felt to lower back at this time. Employee had an increase in back pain over the next three shifts resulting in difficulty walking, standing, and sitting.
Was lifting miner cable to hang it and pulled groin.
Individual was building a wall when he went to put a block up and the block fell striking him on the wrist fracturing his wrist.
Employee was walking between the rib and the single boom. The rib rolled out on the corner striking employee into the platform of the single boom. Hitting the platform cut his right shoulder and a rock came to rest on right leg.
While the employee was repairing the piston of a steering bottle from the 773B when loading it he felt a strong pain in the neck and the back.
Operator was tramming scoop to out side made turn off and slid down on rubber stop and mashed hand (left) between rubber stop and frame of scoop.
While running the continuous miner, a piece of rib rolled out and struck the miner on the back of his right leg and ankle. The employee began missing work on 20 June 2011 as a result of the injury.
Employee was walking down the stairs getting off the equipment, when she slipped. Employee landed on her feet and felt pain in right thigh. This resulted in a Thigh contusion
Attempting to operate loaded truck on an incline when it overturned. This occurred at night. EE had seatbelt fastened and received bruising from belt. Due to size of drive the past was removed from the truck so ee could exit from vehicle. When upright truck started and no damage was done & fan on motor. Drive line was damaged and windshield broke, driver side door also damaged.
EE burned old blasting caps and other refuse on the waste dump on the surface, away from the portal and buildings. He used diesel soaked paper towels to ignite the fire. Upon ignition an explosion occurred which caused lacerations and burns.
The injured EE was helping another EE get posts for wedges for 2nd mining. As EE was unloading wedges from a wagon the scoop being operated by the other EE accidentally bumped the wheel of the wagon which caused it to hit EE's left knee. The scoop operator thought he was far enough away from the wagon.
Employee was hanging continuous miner cable when continuous miner operator took up slack in cable knocking employee to the ground. *Filed under protest due to part 50 audit.
Employee was loading an empty wooden pallet onto an empty rail supply car and he felt a sharp pain in his arm and shoulder. Employee continued to work until 7/28/2011. Company received medical documentation on 9/28/2011 that incident was reportable.

Injured employee was setting in the rear seat operators side of an A.L. Lee 10-Man ride. When the employee decide to put on his long sleeve shirt, while the ride was moving going under an overcast striking his left hand resulting in a fracture.
A gear boy was being sheaved from the stage loader with a steel cable. A chain broke holding sheave wheel causing cable to strike employee knocking him into a built crib.
Employee was changing top roller on belt and when he stepped off belt to ladder he slipped and hurt back groin and right upper leg
In the longwall of 16D, a duckbill scoop operator was backing up and turning. The injured was standing too close and the duckbill attachment caught his right foot between the duckbill and the floor causing a contusion. No issues with training, mining methods, equipment or compliance.
The employee and the employee's personal physicians believe there was an inhalation exposure to a chemical during work shifts in April 2011. The employee believes the possible occupational illness came from breathing tire smoke from a burning tire on the burner floor of the cement plant. A worker comp claim was filed but no occupational illness has been confirmed.

Employee was replacing coolant temp sensor on 992G Wheel Loader. When sensor was removed, hot antifreeze sprayed onto employee causing burns to his pelvic areas.
Victim was assisting with the install of the final drive in D-10 dozer. They were using alignment bars and hoist on boom truck. The weight shifted it caught his hand between drive and undercarriage. Transported to Highland Regional ARH then to UK Medical Ctr. Two fractures and laceration. Treated with cast and stitches & released. Still off work to date.
EE states that while walking into the refinery, he stepped off road to avoid traffic and in the process of stepping off the road he claims he twisted his knee aggravating an old knee injury. The previous knee injury was non work related. Employer is ""Red Lining"" this injury and will contest the validity of the work related status.
Employee was moving trash when she felt pain in her back. She was evaluated by a physician and taken off of work on 6/14/11.

Employee was watching a cable being moved. Cabled whipped, catching the employee around his ankles and knocking him off his feet. Employee fell, landing on right side, fracturing his right shoulder, hurting right wrist, laceration to the right side of his head, and right ear.
Injured was unplugging kiln 2 oxygen analyzer pipe when the air cannon cycled blowing hot dust into his eyes.
Employee was walking on the crusher deck under the wet screens and hit his head on a pipe, wrenching his neck. He was wearing his hard hat.
Employee was working on top of #2 Elev. when co-worker opened the breach door letting dust out in area where employee was working.
Injured employee was assisting fellow employee pull bolter cable slack when he caught his ankle between the rib corner and the cable and was knocked to the ground as it was being pulled by the scoop. Injured employee continued to work until 6/17 when he began to miss work as a result of a back strain.
Carrying a return roller along 3N#2 Beltline when stepped awkwardly on right foot resulting in a distal fibula fracture of right ankle.
The employee stated that as he was cleaning up a rib roll along the belt, a piece of coal/rock he was handling bounced back off the belt striking the employee in the ring finger of his left gloved hand resulting in a fracture.
A production employee twisted or sprained his back while clearing buildup in the kiln system.
He was placing cribs under the pontoons of a shield when the shield slid on the blade of the scoop and caught him between the shield canopy and the rib causing a fracture to his right wrist and hand.

Employee stated she hit a hole in the roadway and jarred her neck, shoulder and lower back. Employee missed first shift on 6/13/2011. This accident is now reportable. Unable to obtain any medical records until E.R. Doctor signed off on initial visit.

While cutting top, a miner bit broke off, striking the injured person in the head, cutting him above the eye.

An electrician was walking down two steps from the mine office when he twisted his left ankle. He did not slip or fall. He underwent surgery at CAMC to repair a broken tibia and fibula at the ankle.
Employee was using a hoist/come-a-long to help place a sprocket on a shaft. The hoist became dislodged catching his finger in between the sprocket and the shaft.
Employee was walking across mine office road when he tripped on left foot, fell to the ground, and sprained right knee. Employee felt pain in right knee and was unable to bear weight on foot.
Employee was pulling a loose rib in the #0 entry; when the first part of the rib was pulled and came loose, the rib behind the employee came loose also, striking the employee on the left hip and ribs. Employee had damage to left ribs and dislocated left hip.

Changing a wrench hydraulic unit out on a drill. The new unit was bolted up and when he stepped to the ground his stomach started hurting. He was diagnosed with a hernia that required surgery.

Ee was striking a bolt with a hammer when a piece of metal went into his eye. The er could not get it out. The eye doctor got it out, no problem. Missed one day of work
Employee was in #3 entry hanging miner cable. As he was hanging the cable he felt pain in his back. Employee was transported to Raleigh General Hospital and was diagnosed with a back strain.
Tramming Joy CM downgrade when the miner began free-wheeling down the grade. The employee, positioned outby the miner, got his feet caught by the outby strands (loops) of the miner cable causing him to fall and be dragged along the bottom for a short distance.
The employee stated that he was bent over and picking up block when a rock fell from the top. The rock hit him in the middle of the back.
Injured was carrying buckets of stopping plaster and felt pain in his back.
He was walking and stepped on a rock and twisted his left knee. He continued to work until he wanted to have an MRI. MRI results showed a ligament tear to the knee. He began missing work on 8/19/11 to have surgery.

He was on the man trip preparing to go underground. The man trip did not go into reverse but stuck in forward. It was parked in front of the maintenance trailer. When the man trip moved it struck the towing tongue of the trailer, the sudden stop caused him to twist forcefully. His left ankle started bothering him 4 hours later, diagnosed with Fx of lower left fibula.
Employee was constructing a stopping when he tripped and fell hitting his lower back on a half block. Employees SCSR was located on his lower back in which is where his pain originated. Employee finished his shift then went to Good Samaritan in Vincennes, Indiana where the doctor ordered him to stay off work for an uncertain period of time.

While attempting to open a plugged air line by taking a quick connect fitting off the pressure on the air line pushed dust into the operator's eye.
Employee was driving, the diesel scoop company #388 motor first outby, when the brakes locked up. This Caused the employee to hit his left shoulder on the steel plate in the cab. A wire had come off the park brake pressure switch which caused the brakes to set.
EE was pushing a wheelbarrow full of cement dust. Was wearing all PPE including safety glasses. Turned wheelbarrow over to dump dust into bobcat, dust came up and entered his left eye. Taken to first aid office where washed eye and placed eye drops into eye, same procedure at 2:30pm on 6/9/11. EE came into off with his eye swelled, red and almost shut, doc appt was made.

Changing bucket tooth on Caterpillar 988F. After removing the pin that holds the tooth, he hit the edge of the tooth with a hammer to loosen the tooth from the adapter when a piece of metal chipped off. The piece of metal struck the right inner thigh of his right leg.
Employee was building a stopping in #6R across from feeder. A piece of rock broke loose from the rib striking employee on the right shoulder and right hand. Employee was transported to Raleigh General Hospital. Xrays were taken and was told to follow up with a specialist for further evaluation.
EE was walking across bench area stepped on a rock causing him to fall, twisting left ankle. Took EE to UAB Medical West emergency for treatment. Emergency room Dr. said EE had a Sprained Joint referred him to his family Dr. Family Dr. released EE to return to work next day

Digging out a indention hole with a hand held pry bar to hook the chain into. Felt a pop in right shoulder. 7/7/11 Ed told Ronda of the accident and he was going to the dr 7/8/11. Ed would need a MRI on his shoulder around the 19th of july. Ed has a torn rotator cuff and will need surgery. His next appt is Aug 4th for consult.
Injured employee was in the sitting position with right leg under him, left leg out straight. He went to get up using his right leg, as he got up his ankle rolled he lost his balance and fell back on his ankle.
An electrician placed his dinner bucket on the ground outside the parts building at the start of his shift. He twisted his back, instead of facing the location he chose to place the bucket. He felt a ""pop"" in his back and had to be immobilized immediately. He was transported to the hospital and is off work for treatment. He has a history of back injuries.

EE was welding on a parts box with his back to another parts box. Two other employees were using an overhead hoist to move the second box and it flipped over striking EE fracturing his right lower leg.
Working on 450 excavator replacing bucket pin and was struck by the boom.

Employee was carrying roof mesh along the left side of the continuous miner, when he stumbled and caught his left hand between the rib and the continuous miner rib protector. Resulted in a fracture to the tip of his left ring finger.
The dozer left the work area and traveled along the edge of the highwall and came to a stop. When the dozer went into reverse, the ground underneath the left track gave way causing the dozer to fall off the highwall. The dozer landed in upright position at the base of the highwall.
Dredge pipeline was plugged, EE turned on the booster pump to clear the line. The EE was standing next to the plastic pipeline when it came apart. The pipeline hit the EE slamming him into the I-Beam on the booster framework. The EE had his leg broken in 3 places and his ankle bone was crushed.

Employee was in the process of untangling a miner cable and water line when he tripped and twisted his left knee. He was taken to a local hospital; x-rays were performed with negative results. Injured employee returned to work and continued to work, but pain worsens. On 6-28-2011 a MRI was performed. On 7-11-11 the specialist removed him from work, to repair the ACL of his knee.
While hosing down top of stageloader with back to rib he was struck by a rib that fell. A piece of rock approximately 10 feet long X 6 feet high X 2 feet thick pushed him against the stageloader gear box resulting in fracture to the right leg and nose and multiple lacerations on his face, as well as burns to both legs.
Miner dropped rock on left foot, initial exam was uncertain -- reexamination on 6-9-11 verified a hairline fracture of metatarsal on outside left behind small toe.
Mine Electrician was burned after attempting to connect a pothead to an energized switch house.

EE walked to fix guard and fell down onto pile of aggregate hurting his left arm, shoulder neck right elbow and left leg and lower back. scraping fingers he was given first aid and refused to go to hospital and continued working. He reported to workers comp on Monday the 6th

Wire way was lowered to the floor and the wire way was in one piece with a flanged connection. EE was taking the connection apart to get it off the floor. When he took the last bolt out the forward piece hit the floor. EE thought that it wasn't as heavy as it was and smashed his finger between the flange and the floor.
Employee was walking on barge while it was being loaded for unknown reason. He claims his knee locked up causing him to fall into the water.
Foot was stuck in mud, attempted to pull foot out of mud when he pulled muscle in left leg. Kept working on it for 9 shifts. When he still had pain he sought medical attention.
Injured was replacing a tire on a haul truck when tire shifted striking injured miner's hand.
Two miners were installing rock bolts for ground support after scaling and bolting the back. As they were drilling a rock bolt hole a large slab of rock slid down from the hanging wall side breaking the timber supports, supporting the two miners. One miner fell 14' to a rock pile and was seriously hurt with broken ribs and collar bone, the other did not fall and was uninjured.
EE alleges an injury occurred which resulted in stomach and chest pain while bending and or installing roof bolts and cable bolts. Note: This accident may be updated EE indicated to supervisor that he is suffering from a collapsed lung and pneumonia. May not be work related.
Injured was operating a shuttle car while it was being loaded by the miner a rock came out of the boom and struck the injured on the right shoulder.
EE was removing 3"" brass gate valves from the speed rack in the bldg. The shelf was 8' off the ground. EE was using a rolling ladder to remove valves. These valves come in boxes of 2 each. As he removed the box of 2 valves he felt a pull in his lower stomach
The truck driver went against company policy by climbing on his trailer to check for loose coal, as he was coming down the steps he lost his footing on the bottom step, he fell on the fender of the truck and then fell about 3 foot to the ground landing on his right hip.
Employee was walking around shuttle car and stepped on a rock, causing him to stumble and twist his knee. After the incident, the employee continued to work until June 16, 2011 when his physician removed him from work.
When Ee removed check plug from the miner head, oil flew out into his face. Oil and metal shavings got into his eye. Missed three days work, but his eye is not going to have any damage.

Extra drill steel fell and hit our employee on the back of the head and left shoulder.
EE was involved in palletizing 100 pound bags when he spun bag around to get a better grip. This action caused trauma to right hand area and thumb was sprained and possible ligament damage but doctor is not sure. He will return to work on light duty on 07/03/2011 for about 6 weeks tentatively.
Employee was loading an oxygen tank onto the welding truck. The tank slipped and smashed his left little finger against the tank rack.
Employee was performing maintenance in inspection speed in the 8' shaft, when the skip encountered a tight spot at the 1040' location. This tight spot caused the skip to momentarily stop and then release causing the employee to lose his balance and fall. He put his rt. arm out and broke his forearm.

He was sitting on a angle iron support frame handing tools to another employee when he slipped off the angle iron striking a frame support tubing with his left knee. Employee was helped out and transported by the supt. To Speaefish Regional Hospital where he was treated and released.
Employee was lifting a 12""x12""x2' piece of wood when he felt a strain in his neck and right shoulder area, continued work rest of the week. Following Monday didn't come to work went to Doctor, Referred to Columbia, MO. hospital where injections were attempted ending in surgery for a herniated disc in neck
EE was changing screens in a 6' X 16' screen deck. He attempted to move the top screen using a bar inserted down through screen. When he pulled on the bar he felt a sharp a pain in his low back. he was diagnosed with a muscle strain.
While dating up faces, employee slipped and twisted his knee. Went to doctor on 06/22/2011 but worked until 07/18/2011. On this date he began missing work in order to have it repaired.
Employee was inspecting the v-belts for the MBH9F fan. He was manually rolling the sheaves and the v-belts and his fingers got caught in the pinch point between the sheave and the v-belt. This equipment was locked out.
The man finished his work week. The man went on vacation the next week. While on vacation he went to hospital he was told hand was fractured he did not return to work on 7/5/11.
Employee was opening an equipment air lock door and came in contact with material being transported through the doors.
When employee was crossing under conveyor he slipped on the wet ground. He tried to catch himself. All weight went onto left ankle when he fell.

Employee was out at the Bldg 16/30 smoking area during a meal break and tripped over a wheel chock staged adjacent to the well lit area for delivery vehicles utilizing the ramp entrance. The employee fell as a result of this trip and injured the lower portion of his left leg.
Employee claims he slipped on loose debris. He says he fell and landed on the outside of his left thigh. The resulting contusion caused swelling in the left knee and lower leg. Employee reported this injury on 6 July 2011.
Pulling stone on roller feed conveyor attached to hydraulic stone splitter the employee shoulder started hurting. When moving shoulder pop could be heard. Possible sprain (r) shoulder. Stone was being pulled as normal operation on roller feed tray of splitter. At time of incident was not reported to upper management. Reported on 7/12/11. Off work effective 7/12/11.
Employee was stepping from platform of roof bolting machine to the walkway area of machine when his foot slipped causing him to lose his balance and fall causing pain in his lower back. Transported to Montgomery General Hospital where x-rays were negative. MRI is scheduled.

He was unloading belt structure from a supply car when he felt pain in his left shoulder. *We dispute the allegations in Citation #8055887 and submit this report solely to terminate the citation.
IW stepped off bolter, planted left foot and turned. Left knee popped and knee gave out, causing IW to fall to ground. Seen at ER, released to restricted duty. Previous non work-related knee problems investigated, MRI revealed new ACL full tear, claim accepted by workers' comp on 07/12/11. Surgery scheduled.
Lifting miner cable and water hose out of cable handler and there was no slack. He strained his left elbow. Employee continued to work until August 3, when he was seen by a panel physician and given restrictions of no use of the left arm. He had a severe sprain.
The employee was moving a stored SCSR Cache and felt a tear feeling in his abdomen area. After consulting with a doctor on the following Monday it was determined that the employee was in need of surgery do to a hernia. The employee was placed on restricted duty.
Putting an agitator in place. Lifting with a winch and shaft of agitator moved. The employee grabbed the shaft to control it and it yanked his left arm.
EE crawled up on the screen plant to remove a rock left from the year before. When he was getting back down off the screen plant he caught the toe of his boot on the step and fell 5 ft to the ground. To brace his fall he stuck his hand out and landed on it breaking the bone above his wrist. This was his left arm.
A large stone on the breaker chain rolled back onto employee's left middle finger. Finger fracture and laceration.
Employee was about,10-20 feet away from the 111 shovel as crew was working on replacing teeth on the dipper. One of the crew was using an 8-12 pound sledge hammer on the tooth in order to remove it. The hammer struck metal and a piece flew off striking injured employee in the left elbow.

Employee picked up a bag containing training materials and felt a twist in her back which didn't present pain at the time of the incident. The next she day wasn't sure if the pain was from cramps or kidney. She did not seek medical attention until June 19, 2011 and she reported the back pain issue to her immediate supervisor on June 20, 2011
Welding a plug on the outside of a chute. The weld broke and a piece of metal came loose and struck the top right of his head causing a laceration. Employee failed to correctly use personal protective equipment he removed his hard hat to weld. Sought medical treatment at Beckley Arh ER received 6 staples in top right of head.
EE was assisting with repairs. Using a wrench, the wrench slipped, ee slipped and fell backwards from a sitting position onto a rock on concrete. EE incurred bruised muscled in back. EE had a preexisting herniated disk in back, He was currently completing physical therapy so ee was taken to hsp for observation. EE was instructed to rest and stretch the muscles.

After the morning walk around with safety chare and tool box meeting the crew then completed stretch and flex. The crew was then instructed to change out used screens on the screen deck. When the last bolt was being tightened is when employee felt a sharp pain in his lower back.
Backed over bolder with loader and jarred his back.
Employee was walking on gravity rollers (taking a short cut) when his right foot and leg rolled through and became stuck.
A mechanic, who was on the work platform at the SAG mill, leaned out against the removable handrail to reach an air duct hose when the handrail came off the mounting brackets and him and the handrail fell to the mill floor. Fall was 6 feet 7 inches from platform. Mechanic landed on his feet, receiving multiple fractures to his heel bone on his right foot.
Employee was removing a pin from the 930E front wheel group when the jack slipped out and struck the employee fracturing his left leg.
Employee was working with a crew construction seals underground. He dropped a concrete block and mashed his right thumb between two blocks. He required medical treatment and received stitches to close the wound.
A forklift was being used to install a pin in a dragline bucket's left drag hitch extension. The forklift operator was using the forklift tines to put side pressure on the pin to seat it. The tines slipped off the pin and contacted a scaffold that one employee was standing on. The jarring motion caused pain in the employee's lower back.
Employee was operating forklift when he turned the corner too sharp and his left foot got pinched between the forklift and Quadromill support beam. Left foot suffered a laceration on the side of the foot extending around the small toe.
Employee was moving miner out of #2 entry on #1 unit. He thought he had turned the miner off but as he reached down to pick up a rope to hang on miner tail the miner moved causing his head to be caught between the tail of the miner and the coal rib.
Employee stated that he was climbing the ladder to get onto a coal loader so as to pull it out of the shop. As he reached the top, he felt a pop and burning sensation in his right shoulder.
Miner was descending newly installed staircase on a sand cleaning plant. As the miner stepped off of the last step, he slipped (ground is made of gravel). As the miner slipped he felt a pop in his right ankle, so he tried to catch himself with left leg. This caused an injury to his knee, and he fell to the ground. Employee will receive an MRI on his knee for an injury diagnosis.
Employee was holding tee board on top of crib when employee using hammer let the hammer slip out and strike employee's right middle finger.

Employee was exiting a front end loader and slipped on the last step next to ground level.
Bolt machine was tramming up entry 3 in rehab area F. Conditions were wet and slick and at an incline. As operator was tramming from deck employee seen cable in a pinch point and reacted by trying to move cable. As he moved cable bolt machine slid sideways and his knee was caught between rib and ATRS pad. He fell and twisted knee. He continued to work for one Hour
Emp was driving the driller pickup south on quarry road. He turned left into the path of a pit truck passing on the left. Impact was on the left front fender and left door.
Employee was walking in gravel when he slipped and fell into a MCC hitting his right shoulder.
Bending down on knee, raised up into standing position and had severe pain in the back, unable to move.
While climbing down of slack pulley structure, employee felt pain in lower back.
Employee was cleaning out a mud ring from the #3 kiln. A section broke free forcing him into a crouching position, injuring his right knee and ankle. 1st visit to E.R. showed nothing. No signs of improvement at follow up visit. 3rd visit showed a little improvement, referred to a specialist. Physical therapy and restricted duty have been prescribed as of 7/13/2011.
Employee alleges he was placing blowbars in crusher and the rotar got stuck snapping the bar while his arm was still holding it causing a strain to his right elbow.
EMPLOYEE WAS SERVICING DOZER #1244 A D11CD. HE SAW NO ANTIFREEZE IN THE SIGHT GLASS AND LOOSENED THE RADIATOR CAP ONE CLICK. ANTIFREEZE STARTED SPRAYING OUT FROM UNDER THE CAP. THEN THE CAP BLEW OFF. HE WAS STANDING ON THE SIDE OF THE RADIATOR AND JUMPED OFF TO THE GROUND. EMPLOYEE BROKE HIS RIGHT ANKLE AND HAD SECOND DEGREE BURNS TO CHEST AND ARMS.
EE was climbing down scraper and fell approximately 4 feet and landed on left shoulder. EE was put on restrictions thus constituting MSHA reportability.
Employee was at the 2nd east washbay. He was taking the radiator cap off. It was under pressure causing employee to slip on the wet floor. He landed with either his scsr, or his cap lamp battery, between him and the ground.
ALLEGED: Employee states he strained his back at approximately 4pm on afternoon shift 6-3-11. EE was either bending down to pick up a gas can OR had just lifted the can when he felt pain. There were no witnesses present. He was evaluated on-site by an EMT and taken to the hospital for further evaluation. SEE FAXED SLIP FOR COMPLETE NARRATION.
Employee took a step and rolled her ankle which resulted in a fracture.
Was walking up catwalk to replace screen on shaker left leg got caught in a bar, bent down to move bar stumbled, took a step & felt sharp pain in left knee. Sprained left knee - went to Dr 6/6/11
EE was performing his regular job on 6/01/11 when a piece of slag burned him, at the end of June ee was still experiencing pain and reported to the office that he needed to seek medical treatment. On June 27, employee was written out of work for medical treatment due to his diabetes not allowing the injury to heal making this a lost time incident

Injured employee was holding a 2 1/4"" wrench on bolt head supporting the middle of the wrench with left hand. He was holding the end of the wrench with his right hand. When the torque wrench was engaged the hand wrench turned pinching employee's left thumb between the wrench and pump pedestal.
On 6/29/11 while closing caps on the Kiln Trunion Rolls employee felt back pain. On 7-19-11 working on the four hole wear plate at the Roller Mill, the pain in his lower back became worse. ON 7/25/11, physician states that EE has damage to his SI joint, recommended restricted duty, physical therapy and medication.
The EE was going under mining cable, lifting sagging cable up to cross under, his left elbow caught top and pulled his shoulder down.
EE was prepping 5 south to start loading holes for blasting. While shoveling to find the bottom row holes, a rock fell from about 3 feet and landed on his hand.
Employee knowingly & willfully circumvented health & safety policies by climbing over a handrail and walking across a dam wall. EE fell off wall and landed on his left side. He was checked out and released back to work. During his follow up the PA-C requested x-rays, which were positive for cracks in ribs.
Employee was installing a new 20HP electric motor on the #3 conveyor. A 3/4 ton chain hoist was being used to lift the motor into place. The safety latch on the hook failed resulting in the motor striking the employees left forearm.
Employee was trying to roll the ring cover over when the pry bar slipped out of the bolt hole. When the pry bar came loose it allowed the plate to roll back over landing on top of the employee's right foot. Employee is on light duty.
EXPERIENCED PAIN IN LOWER BACK AFTER OPENING VALVES ON TANK AT FLOOR LEVEL.
Injured employee was driving a forklift. The tire broke apart and caused the forklift to hit a steel beam which forced employees legs into the steering column of the forklift. Claimant sustained a stained right knee.
Employee was grinding a handrail with a hand-held grinder to prepare for painting. The grinder jerked to the side when he contacted the metal and slammed his hand into the vertical part of the handrail.
Employee was stepping on an overburden truck when pain was felt in knee.
