Accident type, without injuries

Roof fall in #5 entry, 1 brake outby spad #313. 10'high 20'width 60' in length. Entrance to fall was dangered off and supported as per roof control plan and will not be cleaned up.

Electrical problem with the hoist. It was determined we could not fix it within the allowed time. Called the MSHA hotline number to report the accident (downtime) of hoist. Had all miners come out of mine. Repairs to hoist was made within a few hours. Hoist put back into service.

Main fuse blew on Production Hoist. Production hoist and Production Shaft are the second way out of mine. Mine was evacuated. Fuse was replaced and hoist was operational again.

In a worked out section that had been dangered off at the time the head was working. Five entry had been dangered off. A piece of rock between two hill seems fell out. Found at the time of making the returns. About 1400' off of section the entry had been curtain off for ventilation. 10' to 3' wide, 7' thick, 30' long.

An unintentional roof fall occurred on the Unit 4 Outby Feeder Right Return in the Entry 8, between Crosscuts 97 and 98. The roof fall measured approximately 80' in length, 18' in width, and 25' in thickness. A 103 (k) Order (8430282), was issued on April 28, 2011 by MSHA Inspector

A fall was discovered at #4 Mains Sectio (#2 Entry, between #38 and #39 Block). The fall was outby 3100' of the nearest section. The area was bolted with 6' Super Twist bolts. No ventilation, travelways, equipment, or people were affected.

An unplanned roof fall has occurred in the #6 entry on 2nd panel at between spad #1200 and spad #1210. Fall was approx 160' long X 20' wide X 6' to 7'high. Area was bolted with 6' resign bolts. There were no injuries involved and the fall did not affect ventilation or any travel ways.

At 7:00 PM the weekly examiner discovered an unintentional roof fall in the 8 hlg bleeders on the No. 4 break, No. 2 Entry, between 8M and 8L. The fall was approximately 20'x20'x10'.

An unplanned roof fall was found the exact date/time of fall is not known found at approx. 11:30 a.m.The fall is located in #5 entry of the #3 East Mains at c.c. #35 in the intersection. Fall approx. 8' X 10' high and 18' wide. The length of the fall could not be determined. There was no property damage or injuries.

An unintentional roof fall occurred on the longwall recovery face. An area on the face side, in front of the shields fell at the anchor point of the 7' bolts. The area was approximately 7' high x 3' wide x 15' long. No one was trapped, or injured. The escape way was not blocked.

A fall of roof has occurred in the South East mains in # 5 entry at station # 32+20. The fall measured approximately 20 feet long x 30 feet wide and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.

A fall was discovered at on the #3 Mains Section (#1 Entry between #8 and #9 Block). The fall was 7500' outby the nearest section. The area was bolted with 6' Resin bolts. No ventilation, travelways, equipment, or people were affected.

An unintentional fall was discovered on the 1 Northwest panel one break inby spad #526 in the #6 entry. this fall was discovered on a weekly air way examination. The entry width was 21' wide timbered down to 17'4"". The fall measured 20' wide X approximately 60' long X 8' high. the top was constructed of laminated slate and fell to the rider seam.

A roof fall was discovered in the 1Z panel xc-13, spad #9+10 between #5 and #6 entries. The approximate size of the fall was length- 40ft, width- 18ft and height 8ft plus.

At approximately 4:05pm lightning struck water and oil storage tanks at a gas well compressor station site owned by Anadarko Inc located in the DI Mining Area. Two tanks were distroyed and the oil/distillate caught fire. The MSHA hot line was called at 4:42pm.The local fire department was called and the fire was extinguished at 5:40pm.

An unintentional roof fall occurred on the 11rt A belt at 19 c.c. The fall measured 45' X 20' X 8'.

Because of excessive snow melt the screen that filters cooling water for the hoist bearings became plugged which resulted in them overheating. This caused a vibration which resulted in the hoist being shut down until a thorough examination and repair could be done. There were approximately 11 employees under ground who walked to the Coeur or climbed to 3700 then walked to the Coeur

The CM was loading in #6 entry in right pillar cut when a methane ignition occurred near head of machine. Contributing factors to accident are unknown due to accident not being reported by hourly ee to a member of mine mgt until 4/28/11. This accident occurred on a retreat mining section making an investigation of the accident site inaccessible at the time of notification

The scissor hinge on top of the Pleasant Grove elevator / hoist door weld broke loose and fell down the shaft. This allowed the elevator / hoist door to open while in use. Elevator / hoist immediately removed from service. No entrapment or injuries as result of this incident. Elevator back in service at 2:27 PM.

A roof fall was discovered on this date. The fall was located on the 3-North-2 in the #5 entry at the 4th crosscut. The fall measured 40' by 20' and was 6' in thickness. The area was originally supported with 5' Hy-tech bolts. The fall has been cleaned up and re-supported. There were not persons affected.

On 12/20/2011 an unintentional roof fall was discovered in the #4 entry of the #1 Southwest Panel between 49 and 50 break of the scoop road. On 04/19/2011 this area was showing some signs of top activity and we started adding extra support but did not get it completed. We were going to finish it on 04/20/2011. The fall measured approximately 70' long x 18' wide x 9' high.

Unplanned roof fall above the anchorage zone. The area was bolted using 7 foot torque tensioned bolts on 5 foot centers or less. Full coverage roof mesh was installed at the time of development.

The 8 haulage elevator was down more than 30 minutes. There was work being performed on the surface to repair cracks in the elevator shaft. Initial findings indicate that grout spillage on top of the elevator caused it not to operate in call/automatic mode. The elevator would operate in manual mode.

A roof fall was found at 6 right in the Holly Bush Submains at break 17 - 18 in the #5 entry. The size of the fall was approx. 20'W x 50'L x 6'Thick. No ventilation or equipment effected. The area has been dangered off.

Coeur Hoist was down due to trip of safety circuit. Problem traced to a burned (shorted) electrical coil on the brake system. Coil was replaced, hoist back up and running at 1645 hrs, April 14, 2011.

IGNITION: Rock and coal were going through the No. 3 Feeder when a possible ignition occurred in the feeder chain area. A witness stated it lasted 2 seconds and went out by itself. Did not use anything to put it out.

In the No.4 Entry (primary escapeway) between x-cuts No.4 and No.5 going underground from the surface, there was a rock fall measuring 15' wide, 8' long, and 4 1/2' thick.

Unplanned roof fall on #4 belt in the track entry (#4 entry) at spad #707. Fall was approx. 55' L. X 4' Thick X 20' W. Area bolted with 6' resin bolts, no injuries or entrapment involved. Fall was in secondary escapeway and fell on track, high voltage cable and the trickle dust cable.

A roof fall was discovered in the #8 entry of 3 right off #4 N.E. (1 break inby spad # 3578)The fall measured approx. 20' W X 30' L. X 8' H. There was no Eq. or injuries involved and the fall will not be cleaned up.

An unplanned roof fall 20' x 60'-70' long x approx 8' high occurred in the No. 4 entry between xcuts 34 and 35 and extended into the xcut (No. 35) between No. 3 & 4 entries. There was no property damage or injuries. Proximity of soft shale roof, slips & joints and water in roof contributed to the occurrence.

The area was experiencing strong winds. The wind caused 2 overhead power lines to get close and spark. The sparks fell to the ground and ignited dry grass in remote area. The local Fire dept. extinguished the fire at 2:10 pm.

We had a fall of ground in the 6150-15 west stope that took down about 75 feet of the stope. The stope at the time measured about 20 feet wide. One miner is still unaccounted for.

The #1 elevator was out of service for a period of time exceeding 30 minutes due to a broken retiring cam cable. No individuals were on the elevator at the time. All government agencies were notified according to law. The cable was replaced and the car placed back into service @1:20PM. All affected escape ways were re-routed.

The roof in the 3R crosscut (72+00)on No. 2 unit (MMU 002-0) fell out at or below the anchorage in the unbolted crosscut and pulled a few roof bolts adjacent to the unbolted area. The fall occurred around 10:40 PM. It was 10:54 before witnesses felt the area was safe enough to determine that the fall may possibly be reportable.

An unintentional roof fall occurred @ 52+50 (x-cut 86) on the 5B travelway. The fall measured 20'x 22'x 7'. The fall did not affect ventilation or cause personal injury. The fall did affect passage of men because the fall was in the secondary escapeway. The stopping line had to be set over to make a new travelway.

A roof fall was found today in the bleeder entries where 3 left panel had been cut into 2 left. This fall was called into the MSHA Hot line at 4:33 p.m. There was no entrapment, and no apparent life threats. Fall was 1 bk inby and 2 bks outby spad # 7560. Width and Height is unknown due to this being a pillar section.

Experienced an unplanned ignition of coal in #2 Ball Mill. At 9:24 a.m. power was lost to the fuel system which prevented the system to be purged as required by the shutdown SOP. The explosion occurred when the mill fan was restarted started. No injuries or damage occurred.

An unplanned roof fall was found on 04/11/2011. This fall was found one break inby survey station No. 3361. This fall measured 20' wide x 20' long x 8' high. This fall does not impede travel. This fall has been dangered off.

A unplanned roof fall occurred in the #3 3ntry. 1 break right from spad# 3556.This fall occurred 11,000' from the active face. Ventilation and passage of persons was not affected. Area will be re-hab'd. Fall meas. approx. 12' L. X 20' W. X 4' H. Area was supported with 4' resin bolts. Federal and State agencies were notified immediately.

A roof fall was discovered in the 3rd Northeast Submain #4 entry, xc-24 spad #19+60. The approximate size of the fall was length-30ft, width-18ft and height-6ft.

Roof initially bolted with 4' FG Resin Bolts. Cutter started working along left rib of L-2 Entry on 4/5/11. Supplemental supports added at that time 6' Supertwist TT Bolts and 12' cable bolts. A 20 foot unbolted cut fell in and the entry outby was center cribbed. Roof fall extended another 20' bolted with 4' FG Resin bolts. (40'L x 19.5'W x 4.5'H).

A roof fall occurred beginning at spad location No. 3880 and continuing through spad location no. 3875 in the last line of crosscuts to the face. The fall measured approximately 175' long x 20' wide x 6' high. A continuous mining machine and roof bolting machine are inby the fall area. The were no injuries.

A roof fall was discovered in the #1 entry of 3-North-1 at the #11 crosscut. The fall measured 20' by 20' and was 6' in thickness. The fall was originally supported with 5' Hy-Tech bolts. The fall will be cleaned up. There were no injuries.

The hoist gears were stripped to nothing when lowering a scoop and slack in the rope tightens it puts alot of tension on the gears of the hoist.

A roof fall was found at 5:40am on 4/4/11 at cross cut 100 entry 4 main south. Fall was 20'wide, 55'long and 12'high. Area had been mined 10/14/1991 and was supported with 5' full resin pins. No injury resulted however 100' of hi-line was lost. There are currently no plans to clean up this fall.

Silver State Wire Rope arrived on the mine site to conduct semi annual rope test on the Hart Street Elevator ropes. At the conclusion of the test mine officials were notified of a 14.3% loss. The hoist was taken out of service and the secondary escapeway was rerouted to the South Portal. All employees have been notified of the change.

A roof fall was discovered in the 4P panel xc-11, spad #7+10 #4 entry. The approximate size of the fall was length- 19ft, width- 20ft and height- 61/2ft.

A roof fall occurred in the 7-Butt Section outby in the #4 entry of the section (30' long x 18' wide x 10 - 12' high). The roof had been supported with 6' FG Resin Bolts and 8' Cable Bolts. Ventilation was not affected. No personnel or equipment was affected. Area comprised of sandstone channel/slicken-sided roof rock.

A roof fall occurred in the 2nd Southwest mains, crosscut #35 in between the #5 and #6 entries. The fall measured approximately 30'x 17'x 6'. The area was supported with 6' tension rebar roof bolts and 8' double lock mega bolts.

Hoist experienced problems with solenoid on actuator that controls drum brake, causing hoist to not operate at all times. Men were withdrawn from mine until hoist was repaired at approximately 11:55 PM.

Roof fall was found on a normal weekly exam of the Box Cut Intake airways. Block 12 @ spad #3677 ""old #1 section track entry"". Fall is in an intersection 18' wide, 25' long, 4 1/2' thick. Bolted with 4' resin bolts - Bolts pulled out approx. 6"" sandstone main roof - Slate top fell.

An unintentional fall of roof occurred in the #6 heading, at or near spad #5518 on the 2nd southwest mains. The fall measured 25' long x 18' wide x 6' to 8' thick.

This fall was found 1 break inby survey station 162. The fall measured 4' thick x 20' wide x 20' long. This fall was found by an EE and was reported by telephone on 04/02/2011. The fall has been cribbed and dangered off. This fall does not impede travel.

An inundation was discovered in the bleeder system of the mine which caused an obstruction in the ventilation system of the longwall. The bleeder fan was shut down to prevent damage to the fan. The mine was evacuated and electrical power was shut off going into the mine. No injuries or equipment damage occurred as a result of this accident.

An employee was struck by a slab of rock that measured approximately 8 inches to 12 inches thick by 50 to 70 inches wide by 74 inches long. The roof fall occurred at the 34 Main between crosscut 1 North 4 and 1 North 5.

A ground/roof fall occurred in the 6150-15 West Stope completely burying an employee under 30 feet of rock.

EE was setting feeder on to tail piece on the 002 section. While cleaning out the gob underneath the cats to put the cribs in place the EE slipped and fell forward. EE put his left hand on the raised jack to catch himself. At this time another EE let the jack down and pinched the EE's left ring finger between the jack and the frame.

EE was assisting to assemble a HP 500 crusher, tightening the screw down, using a shop crane hoist and drive. The drive went under the screw and employee's right middle finger was contacted between the wrench and the drive, causing a laceration to the finger.

Employee's left thumb was caught between a metal liner and a magnet.

Stated: Employee was bolting in the #5 entry in unit 3. He had drilled his first steel, had his bottom steel in the chuck and was attempting to join the two when his index finger on his right hand was caught between the top of the second steel and the mine roof.

Employee was adjusting a scraper bar on a conveyor while it was running. Conveyor grabbed scraper bar and left middle finger was caught between conveyor and scraper bar. Severe laceration to left middle finger requiring surgery and removal of finger tip at first knuckle.

Amputation of index, second, and third fingers on right hand/first joint up.

Caught in, under or between a moving and a stationary object

Employee was installing roof bolts and stated that he placed his left hand on the roof bolt plate to align it and applied the up-pressure at the same time. His left middle finger was caught between the plate and mine roof resulting in the amputation of the tip of his finger.

EE was operating a roof bolter. Attempting to put a bolt through a piece of 3"" X 5"" wood and while drilling the hole ee had hand on drill steel and in the process drilling it jerked his pinky finger off on left hand.

Employee caused pinch point by reaching through hole in workbench table. Approx 1,025lb part was pushed for hole alignment while trying to bolt on rigging for mobile crane lift. Part and table caused a pinch point which amputated the distal Phalanx of the left index finger.

Employee caught left index finger and glove on drill bit. Before drill was shut off it twisted his glove and hand around the bit and amputated his index finger at mid-knuckle.

Struck by falling object

EE was cutting free a section of suspended catwalk, the welders hand was struck by the falling catwalk's handrail, severing 2 fingers. One finger was totally severed the other was partial. The other finger was re-attached.

Employee was transporting HDPE pipe on a Boom Truck. He stopped to re-secure the load which was tied down with a chain binder. As he was retightening the load the cheater bar slipped off the chain binder handle and he was struck in the face resulting in a fractured mandible (rt side)

Employee was installing pogo sticks behind the line curtain when a piece of draw rock fell striking him on the base of the neck and shoulders.

Employee was on the van getting ready to return home at the end of shift. A co-worker left the van door open and it was very cold outside. EE went to close the van door and stepped to the edge of door. She lost her footing & slipped out of the van face first, landing in the parking lot.

On 4/27/11 the EE was operating a Shuttle Car and hit a hole in the roadway causing him to bounce his head on the canopy. He cont'd. to work his regular schedule until 5/24/11 when he went to the Dr. because he was still in some pain and the doctor took him off from work.

He jumped from the platform to the ground landing on both feet but immediately fell to the ground unable to get up. He was diagnosed with a fractured tibia.

IE was standing on a conveyor catwalk when the conveyor frame broke. Sudden drop of the conveyor caused the employee to fall. EMP. HURT LEFT ELBOW WHEN HE TRIPPED OVER A GRATE AND FELL AGAINST THE SIDE OF A TANK. EMP REPORTED THE CLAIM AS RECORD ONLY ON 4/20. ON 5/4/11 EMP REPORTED THE ACCIDENT TO H&S DIRECTOR SO A DOCTOR VISIT COULD BE MADE.

Struck against a moving object

Employee was driving over bumps on dirt road in large haul truck and complained of a soar lower back at the end of his shift.

Employee was closing a swinging air lock door when the ventilation current pulled the door from his control resulting in the employee twisting his knee.

Fall onto or against objects

The employee was walking up the belt line tracking the belt. As he was walking behind the pillar at the midsection. The employee's right knee buckled and the employee fell to the ground as he fell he struck his Thigh on a metal bar to the belt line causing him to bruise his right thigh.
