Accident type, without injuries

An unintentional roof fall was found at Bk 65 in the #6 entry. Roof fall is 1 Bk inby survey spad #4909. The fall is approximately 20'L x 18'W x 5'High. No interruption in ventilation occurred. No injuries were reported. No equipment was involves. Area was dangered off. Fall was found at 7:30am. MSHA hot line was notified. MSHA inspector issued a K order at 7:45am.

During a weekly examination a roof fall was found in an abandoned Sub 1-1st Left Panel. The fall was found at x-cut 43 entry 4. The fall did not affect passage of miners, impede ventilation, or cause injury to miners.

An unintentional roof fall was found while traveling the intake air course. It is located at or near spad #157. It appears to be approximately 20ft. x 30ft. x 10ft. 1st. North West mains, between #7 and #8 entries.

Employee was trapped in T-2 Elevator traveling at the Balling Floor Level. He was trapped for approximately 50 minutes. MSHA was contacted at about 11:15 AM. No injuries occurred.

Smoldering coal was believed to be in the VF 4 of the coal silo. Plans to drawn down the silo at its normal rate while monitoring the temperature of coal drawing the silo was implemented. Once coal silo was near empty water was applied to the coal to eliminate the hot spot of coal. The coal had not fed from VF 4 for quite a while. The hazard was eliminated without incident.

Rock fall occurred on the 1 West Mains 1 1/2 crosscut inby spad 2497 # 1 entry. Fall measured approximately 20' long X 20' wide X 5' thick.

An unplanned fall of the mine roof occurred on the 14 Left Longwall tailgate. The fall occurred at 3+87 in the tail entry for a distance of approximately 30'. The fall was not above the anchorage zone of the bolts however id did impede the passage of people off the face into the tail entry. Ventilation was not affected on the longwall face.

A planned fall in the gob caused an air displacement that knocked down the stopping at crosscut 32 North mains between 5 & 6.

Due to a rain event earlier in the week the Brazos river began to rise substantially. The river eventually rose to a height greater than our levee and entered our dredge pond. No one was exposed to any danger associated with this event. No equipment was damaged as a result of this event.

While attempting to cut rock from the roof to ramp up over an existing fault in the Number 2 entry, the extendable exhausting line curtain was being pressed against the rib rock protruding out from the rib. This condition caused the ventilation to be choked down making it difficult to ventilate the face.

A hoisting control breaker tripped resulting in an hoist outage. Cause of trip believed to have been unusually wet conditions and humidity. Miners were evacuated at 10:26am. and out of the mine at 10:42 am. The control breaker was tested and reset. The hoist was tested and miners returned U.G. at 10:45a.m.

During the production shift The intersection on the Number 1 Section Number 8 entry fell in the last open break.

A roof fall occurred in the intersection of the 3 right #6 room 136 feet inby survey station 3675. The fall was approximately 22 feet long x 22 feet wide x 10 feet high.

A rock fall was found while checking pumps inby spad #0+50 to 1+00 in #7 entry measuring 30ft. long x 18ft wide x 8ft high. Area was supported with 4ft fully grouted roof bolts, 8ft cable bolts and some cribs. This area was mined in October 2011.

A fall was discovered in the #4 Mains Section (#20 Crosscut, #11 to #13 Crosscut). The area was bolted with 6' Super Twist bolts and 12' cable bolts in the intersection. No equipment, ventilation, or people were affected.

Fall was found on 20 break 3 left on 3 left B mains. Fall was 100' long/ 9' to 10' thick, and 18' wide. Roof support was 6' torque tension bolts.

On Tuesday 1/24/12, a roof fall occurred on the 003 pillar section. The fall occurred in the #4 entry while cutting the last run from the left pillar block. The fall caught the head of the miner. A call was made to MSHA and also reported to the 800 number. MSHA inspector came to the mine and was present when the miner was pulled out. No injuries occurred during this incident.

6"" of rain fell within a 3 hour period. Ditches in mine were unable to contain the torrential rain causing an inundation of A-Pit. This flooding was not localized at the mine. There were many local roads and property that were affected by this record rainfall event.

Towards the end of the shift the Dynamic Breaking Contactor on the hoist was not working properly. This shut the hoist down creating an evacuation situation due to not have 2 escape ways to surface. Mine Evacuated with no injuries or other problems. The contactor was cleaned and the hoist was back in service at 13:00 HRs.

Hart Street Portal elevator computer experienced a malfunction. The computer was unable to determine the location of the elevator in the shaft. The computer was reset and the problem was corrected. A functional test run was performed and the elevator was returned to service

A roof fall measuring approximately 8'x18'x12' occurred during an idle shift. The fall was located in the No. 2 to No. 1 crosscut. 1 Crosscut inby spad No. 362. *The fall was discovered during the pre-shift exam on 1-23-12.

The Crabapple elevator was not operational for an extended period of time. The Electrical door switch on the bottom of Crabapple shaft needed adjusted. There was no physical damage to the elevator, no injuries and no entrapment as a result of this incident.

An unplanned roof fall has occurred on #1 Section Sycamore #2 Belt 26 break - 2 breaks inby #5279 spad. The roof fall was approximately 20' x 20' x 4'. No injuries were report and no equipment was involved.

Haul truck 450 drove up a berm causing the truck to flip on its right side. The operator was unhurt. Accident is still under investigation.

The Dotiki 4 escape hoist rope revealed a suspected area from our x-ray examination. Immediately the escape hoist was shut down (removed from service), the complete hoist rope was replaced with a new rope.

We experienced a rock burst in the 190 raise at approximately 6:13 Am. The area was inspected at 7:15 by the mine foreman and he notified the Safety department at 8:07 that there was significant damage in the raise. Then he barricaded the affected area. There was a displacement of Timber and a significant amount of rock down blocking raise. Event occurred at blasting time.

An unintentional roof fall was found during the preshift exam in the intersection of the 7th cross cut and No. 14 entry. The fall was approx 18' wide x 10' high x 20' + long (exact dimensions not known until clean up). MSHA was notified and the approaches timbered off. No injuries, property damage, or limitations to travel resulted.

The hoist car derailed for a period of time that exceed 30 minutes. The hoist car was descending the slope and derailed approximately 200"" down. No injuries were reported.

A roof fall occurred in the north mains at the 59 x-cut, number 8 entry for a distance 16 feet just in the intersection. This did not impede travel, effect ventilation, or result in injuries. It did fall above the anchorage zone. Travel routed around fall.

At 8:30am a small fire was notice on the coal pile south of the shop at the Tatum mine. The MSHA hot line was called. The fire was put out in 30 minutes, the rest of the hot coal was loaded and sent to the power plant.

Contractor (ID#LUO) was performing work by changing out clogged pipe lines that fed the Raconite holding tank. A cutting torch was used that set off vapors within the pipe that let to overpressurizing holding tank.

Roof fall occurred in the 4-East section one cross-cut inby the RHS tail piece. The fall measured 24' long x 18' wide x approximately 12' high. The area was bolted with 6-foot FG resin bolts on a 3' x 4' pattern with large bearing plates and 8-ft to 12-ft cable bolts. A coal rider was detected at 12-feet.

A tree fell across an Avista power line that feeds the Galena mine during a storm which produced extremely high winds. No one was riding the shaft conveyances at the time. Crews were asked to stand down until the power was restored by Avista crews at approx. 6:45 P.M. Hoistiman had no problems restarting equipment.

The Shiendler elevator became inoperable at approximately 10:50PM on Jan. 18, 2012. No one was injured or trapped. Troubleshooting began immediately and a faulty bottom door switch was found and repaired.

Damage to the south skip in the #1 shaft wouldn't allow the skip to close normally, causing the skip to become stuck in the bottom of the shaft. The mine still maintained two means of egress with the #2 and #3 hoists.

The #4 entry intersection 40' outby the face fell 18' X 18' X 12'. The #2 Roof Bolter was in by the last open X-cut when it fell entrapping the machine at the face. The bolter operators escaped without injury. There is no rock on the roof bolter. The fall will not be cleaned up. We will back cut the #4 entry to retrieve the roof bolter.

2-man shaft crew inspecting production shaft when conveyance stopped near bottom landing. Management elected to investigate cause before attempting to retrieve personnel. Resulted in 2-employees isolated on conveyance for over 30-minutes. Guiderail was found to be slightly displaced.

An unintentional roof fall above anchorage point was found by a fire boss while performing Pre-shift inspections. The fall occurred on the deactivated MMU of 006 of the number 1 west mains, the fall was in the number 12 entry intersection approx. 1 crosscut outby the face, spad number 3511

An employee got trapped on the portal passenger elevator at approximately 9:42 PM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The employee was freed and the elevator was returned to service a approximately 2:40 AM. A smashed lady bug had lodged itself at the bottom of the lock enclosure and is believed to have caused the problem.

Lost 2300volt and 480volt power to the Galena hoist due to blown fuses. It was determined that the fuse holder was defective so it was replaced. Our primary hoist is the #3 which was not affected so hoisting of the underground crews took place on schedule. This problem also affected power in the office and dry area. Hoist was back on line at 3:50 pm.

While loading blasting agent onto the blast hole the blasting agent caught fire, detonated and caused the detonation of additional primers that had been placed around the blast hole area. Fire was caused by Blaster smoking near blast hole. See attached MSHA Citation #8521062.

Upon inspection of new porthole, Mine manager found approx: 2 ton of material had slabbed off the face and into new established porthole pulling down a bolt. Mine Manager called immediately reportable hot line at 6:11 am and was on hold for 11 min and 55sec. At this point he hung up and recalled and got through to some one and reported his finding.

On Wednesday, January 11th the #2 kiln and coal mill system were being shutdown to replace the tangential primary airline. The kiln operator began to lower coal feed rate tonnage as is normal procedure in order to empty the coal mill. When the coal mill feed rate was finally showing zero tons per hour the kiln operator received an alarm that the west explosion vent opened.

A roof fall occurred on the 1st 54 supply road between x-cut 21 and x-cut 22 in the pillar line. the fall measured 40'x18'x5'. the fall was caused by a hidden slip in the roof. the area was mined in August 2005.

A fall occurred in the #7 North Section (#4 Heading at #9 Block). The feeder was involved in the roof fall. The area was dangered off. Post and cribs were set around all four approaches to the fall area.

We were testing our Emergency Hoist and doing a trail run with the emergency cage. The cage door was not shut completely and caused the cage to get hung at about 20 feet from the bottom sump. When it came loose the cage dropped to the sump causing minor damage to the door.

A unplanned roof fall occurred in the 1st panel in the #7 entry at spad 1686 in the return air course. Fall is approx. 5,700' from nearest active section. Fall will not be rehab. Passage of person and ventilation was not affected. Fall meas. 160'X20'X4' area was initially supported with 5' torque tension roof bolts. Federal and State agencies were notified immediately.

A roof fall was observed at the enter section of survey station # 1588 in the # two entry of the left return of Byrd 2 Mains airway. Ventilation is not impaired. The # one entry is open for travel and ventilation. Area will be timbered off.

An unplanned roof fall has occurred at 1A Head intake between 5 and 6. It was located one break outby spad 8929 and 8928. The timbers and jacks were set and the roof fall was dangered off. The water line and power was rerouted. No injuries were reported and no equipment was involved.

IGNITION - occurred while burning the insert out of a miner bit lug when a spark ignited a methane bleeder on the inby rib. The flame burned 10 seconds and was extinguished with (1) one fire extinguisher.

Roof falls were discovered in the 8 R panels SE Mains 10,000' from the working section. Failure occurred due to laminations above the anchorage zone of 5' torque tension roof bolts. The are was developed 3/11/11. ventilation and the transportation of miners' were not affected. No injuries occurred as a result of the fall. #8 Entry 20 Break 2 breaks outby 66423, 50'LX19'WX8'H.

Smoke was discovered at the #1 silo at the river loadout. the smoke indicated that a fire was present in the silo.

The Dry ridge Elevator was not operational for more than 30 min. The rope grabs on the top of the elevator shaft needed readjusted. Nothing on this elevator was damaged nor was there anyone trapped or injured due to this event

An unintentional roof fall occurred on the Main West 3 Return, Entry 7 Crosscut 180. The roof fall measured approximately 30' in length, 17' in width, and 10'-12' in thickness.

An unintentional roof fall occurred on the Main West 3, between Entries 5-6 Crosscut 179. The roof fall measured approximately 60' in length, 17' in width, and 7'-8' in thickness.

The Couer hoist went down due to an electrical problem while hoisting ore from 3400. The electricians had a little trouble identifying the problem but finally determined it to be a burned coil and fuse on the secondary board of the hoist controls. The only crews affected were the diamond driller and cages who instructed to stand down. Hoist was back up at 3:30 pm

The hoist which transports supplies and men into the mine at the battle run portal was transporting a supply car into the mine, when the car reached the bottom landing, the cage caught the bottom landing setting the safety dog. this event damaged three guide wheels and the cage was out of service for more than 30 minutes.

An unintentional roof fall was discovered on the #2 Section in the #2 Entry Outby Spad 99. The fall measured approximately 8ft. long x 6ft. wide x 3 1/2ft. high. No injuries resulted from this occurrence.

A roof fall occurred in the old works (in rooms) in 5th Right off 2nd North West Mains at spad No. 1364 No. 6 Entry. The fall is approx 30 to 35 ft long, 20 ft wide and 4 1/2 ft to 5 ft thick (Ht). The fall will not be cleaned up.

At 11:00 pm on January 05, 2012 the service hoist was removed from service on a planed maintenance repair work. At 11:00 pm work started on the replacement of the service hoist motor.

While hoisting a haulage truck bed to surface a power cable that controls hoist function was damaged. Electricians were called to make repairs. This hoist went down at 9:45am. Hoist man tried to call the 1-800-746-1553 twice with no luck. He then called Fletchers Hoist man to report the problem. Fletchers Hoist man called MSHA at 10:35am. to report West Forks downed hoist.

The Dry Ridge Elevator stopped in the shaft which entrapped one miner. There was no damage to the elevator. The elevator stopped due to the door switch on the bottom of the shaft vibrating open. There were no injury as a result of this event

A roof fall occurred on 1-5-12 in 2nd North West Mains at spad #1341. The fall is approx 30 to 35 ft long, 20 ft wide and 4 1/2 to 5 ft thick (no injuries) and the fall occurred in old works and will not be cleaned up (in #5 Entry)

A roof fall occurred at the 7 North shaft C approach. The fall was approx. 10' H. X 20' L. The fall did not interrupt ventilation or walkway. Area dangered off. This is not an active area of the mine.

Control Rm received high CO alarm and Operator was called out to assess. Operator utilized a gas meter to assess and it triggered high CO levels. Coal barn was assessed. Coal barn showed signs of smoke. Fire dept was called, they provided services and fire was extinguished. On-site personnel monitored area. Investigation shows a damaged electrical cord on the C1 Conveyor.

An unplanned roof fall has occurred, above the anchorage, in a previously rehabilitated area, 2000+ outby the working section, in the N0 3 entry, approximately 30 feet outby survey spad 1417. The fall was approximately 80 feet long, 20 feet wide and 5 foot high. Ventilation and/or the transportation of men were not affected.

An unintentional roof fall occurred at x-cut 93 between 1 and 2 entry of the Tailgate Unit 002. It is not blocking the escape way or the ventilation off the unit.

No. 8 Elevator was out of service for a period of time exceeding 30 minutes. The car was not leveling at the bottom landing. The leveling switch was adjusted, the car was tested and placed back in service at 9:00 am. No individuals were on the elevator at the time.

A roof fall 7'L x 19'W x 8'H was discovered outby spad 66570 #4 entry crosswise to the #6 entry. The fall was caused by failure above the anchorage zone of the 5' torque tension roof bolts. The area was developed 4/6/2011 and was 10,000 feet from the face area. Ventilation and the transportation of miners' were not affected. No injuries occurred.

Mantrip had taken men to their equipment and parked up at about 6:15 am. EE was in his hoe and noticed steam or smoke about 6:30 from under the van and went to check on it. It was on fire so he called for help on the radio. Several other operators responded with fire extinguishers. Van was a total loss. No one was injured as a result of the fire.

MMU-002 #2 Entry x-cut 31 & 32 4 North B, a non-injury methane ignition occurred while EE was drilling a 12' cable hole. The drill steel contacted a roof mat and ignited methane inside the drill hole. The flame was put out with water and fire extinguishers.

It was found that a water pipe attached to the side of the Meikle shaft had a missing clamp and loose bolts. Barrick took precautionary measures by shutting down the shaft.A 3rd party structural P.E. was used to determine if it was hazardous, it was confirmed that the pipe was secure and not hazardous. This form was filled out under advisement of the MSHA field office supervisor.

A roof fall measuring 50' L x 19' W x 8' H was discovered in the last open line of crosscuts between #6 and # 7 entries, 50' inby spad 68152. The fall occurred due to failure above the anchorage zone of 5' torque tension and 12' cable roof bolts. Ventilation and the transportation of miners' were affected. No injuries occurred as a result of the fall.

A roof fall was discovered in the #9 seam in the 4S panel xc-21,#7 entry at spad #14+36. The approximate size of the fall was length-40ft, width-18ft and height 6ft plus.

4701 #1 Hoist had computer control malfunction. Electrician's had to re-boot the computer system to get Hoist controls functional again

Drowning

While operating a skid steer loader in reverse, it overturned in body of water.

Struck by flying object

The IEE was operating a supp. motor and was helping move a shuttle car. Shuttle car caught a fire valve and broke the fire valve off. The damaged fire outlet was isolated from the main water supply before repairs were started.The IEE was helping re-install the fire valve when the valve blew off and hit the EE over the lt. eye and forehead, causing lacerations,fractures.

The employee was attempting to remove a large piece of liner steel from the belt to drop it into the steel bin next to the belt. In the process of trying to maneuver the steel by hand, the st eel rolled crushing the tips of three fingers on his right hand. This resulted in an amputation of the tips of his three fingers.

He pulled chain and the chain slipped out of spreader and caught his left index finger in link of chain. Moving 24-10 915 Eimco out of surface diesel shop, pinched left index finger between steering ram and hydrlic hose. Laceration and smashed finger this resulted in amputation of finger tip below the first joint.

EE had a hernia operation approx 2 years ago. On 1/6/12 he was moving some L-3 communication material. He was lifting a L-3 battery pack onto the side of his ride when he felt a pain in his stomach. Went to the doctor and they said he tore his previous hernia that he had. The first day of work he missed was 1/9/12 and is still off.

Two(2)Ironworkers were working on top of a monorail structure. The structure twisted and collapsed when they attempted to reposition beam. EE # 2 was thrown into Silo #1 before landing on ground. EE transferred by EMS to local hospital where he was diagnosed w/a fractured neck causing paralysis. EE was then trans. by med flight to treating hosp. for surgery.

Walking from miner back to load center when he stepped on a rock submerged in water, twisting left ankle and felt pain in his foot. Original RPI 1/31 diagnosed with a fractured foot 2/23.

While in the process of drilling test hole employee placed his left hand on glue tray, a piece of falling material struck the tip of his left ring finger resulting in a avulsion to the finger tip that required the finger nail to be removed. A physician placed him off work due to risk of infection.

Equipment Operator had been operating Caterpillar 627F Scraper to strip overburden. Operator had been operating scraper w/out using ""cushion hitch"" feature. At end of shift on 1/31/12, he spoke of sore neck to co-workers. On 2/1/12 he went to Doctor. Doctor's diagnosis is of a cervical strain and for him not to operate a scraper until some physical therapy could be performed.

Nurse called to lab and informed employee on bathroom floor for an undetermined amount of time, when she called supervisor for assistance, as she reported attempting to lift self off toilet, and resume standing position, when she felt bilateral legs tingling, she then stood up, twisted left ankle, resulting in fall to ground.

After our investigation and doctor reviews, it has been determined that the employee injured his right knee while pulling power cable. The employee was working his normal job until 6/15/2012 when he had surgery on his knee.

EE was greasing the Right Miner when a 10"" long 4"" wide and 3"" thick allegedly fell from the top striking EE on head. There were no witnesses to this incident. Note: this EE has only worked one day at this mine .

He was treated by M.E.T. on the section and transported to Hazard A.R.H. hospital.

As the loader operator dumped into a 777 Cat. truck, a rock from the bucket hit in the truck bed and the driver backed into the loader bucket causing a strain to his back. He has been on restricted duty for four days and is working now at full duty.

Working in the BMX Mains section, the foreman was walking around a parked ram car and inadvertently walked into a rib strap that was damaged and bent outwards striking him in the right temple area causing a laceration. Employee received eight sutures.

Employee reported on 1/25/2012 that while driving a CAT 785 Rock Truck he hit a bump in the haul road causing him to bottom out on his seat resulting in a lower back strain. Following a complete review of the incident, management was unable to determine validity of the report and is filing this 7000-1 according to Part 50 recommendations to report questionable injuries.

The South Sag Mill was being raised using 4 ram jacks and contract employee was shimming the lifting cradle when one of the jacks failed and blew apart. The flying debris hit the employee.

Employee was loading rock dust on scoop. As he turned he felt pain in lower back. Employee was treated at MedExpress.

Caught in, under or between a moving and a stationary object

EE was removing lower air chamber section of 10' air slide with another EE onto a forklift. EE's thumb was caught between the fork and air slide when it was unexpectedly moved by other EE. EE had nail removed and sewn back on as a precaution. EE working with restrictions.

Struck against stationary object

Stepped out of powder truck on to crib block, it rolled and ankle twisted.

Individual was loading bulk truck. As he stepped back he heard a pop come from his right knee and felt a shooting pain. Individual was diagnosed with a tear to his ACL and medial meniscus on 7 Feb. 2012

Fall onto or against objects

The employee was operating a roof bolter and as he reached for a roof bolt he tripped over a rock and hit his elbow on the machine.

Struck against a moving object

An employee operating a Komatsu dozer was pushing an ripping material on a downward grade, somehow the dozer slid sideways and continued to slide 35 feet off a 27 ft high embankment. The equipment rolled one revolution and landed back on its tracks. 911 was called, and then the employee was life flighted from the scene to the University of Utah Hospital.

Struck by falling object

Employee was bolting on MMU003 in #1 entry, spad #2770, when a 28""x4"" piece of draw rock fell from the roof onto his right hand. The height was about 6 1/2'. He received a fracture to his hand and was placed on restricted duty.

Fall to the walkway or working surface

Employee tripped on a piece of rock and fell. He continued to work, had an MRI, and started working with restrictions on 2-24-12.

Over-exertion in lifting objects

On 1/29/2012 employee lifted a partially full bucket of grease and noticed discomfort in his shoulder/chest. Employee reported to employer on 2/2/2012 and was seen by a doctor on 2/10/2012. Diagnosis is an injury to the chest wall.

Fall to the walkway or working surface

The employee was getting out of the parked van at the end of the shift. As he was walking toward the office door, he slipped and fell on his left side, upper arm area. He was evaluated by a Physician after alleging continued problems. Physical therapy was ordered on March 20, 2012 and a series has been completed.

While getting the rig ready to move, the drill assistant was removing the blocks from under the jacks. When he went to pick up the block, he felt something pop in his lower back.

Struck against a moving object

Driving haul truck, swerved to miss a rock, hit rib, which caused driver to hit his head against a nut on the window frame.

The employee stated he felt his back tighten up while moving approximately 40 pound cooler grates from the fabrication table to a pallet. He will receive PT for a strained muscle.

Contusion/Cellulitis - Right Middle Finger - Walking to work area, carrying tools, tripped over torch hose, fell down, 8 lb sledge hammer fell on hand.

Over-exertion in pulling or pushing objects

Employee was pulling jack leg out of a hole and felt a pop in his left elbow.

Struck by falling object

A piece of rock, 8-10 inches in diameter, and 1-2 inches thick, fell, striking EE right hand, cutting his middle finger, while he was attempting to retrieve his drill steel from drill hole.

Temporary staffing employee was using a razor knife and cutting excess tape from a vacuum hose when he accidentally cut his left thumb. He sustained a superficial laceration to the left thumb.

Two miners working to remove a guard from a apron feeder for repairs/clean up. One miner was attempting pry off the guard, while other miner was using an 8 lbs. sledge to strike on the main frame. The injured miner moved to get a better position. The second miner, using the hammer, the hammer glanced off of the frame and stuck the second miner on the right knee cap.

Employee was moving a valve and it fell onto his foot lacerating and contusing right metatarsal area of foot

Employee was trying to guide two large stones as they came off the saw. While guiding the stones, he strained his back. Restricted duty assigned. **Employee did not seek treatment until 2/8/12**
