An unintentional roof fall was discovered near spad number 4800. The fall measured approximately 20 ft. wide x 8 ft. high x 40 ft. long. No injuries resulted from this occurrence.

Inspection of production shaft revealed a missing section of the west center guide rail about 8' above the bottom service landing. Damage guide rail interrupted operations for more than 30-minutes
A roof fall was discovered at the 7th cross-cut inby the Pathfork portal in the No.2 entry. The fall measures 20'x 20'x 3'-5' thick.
A roof fall occurred at x/c #80 in the 2nd Southwest mains at x/c #80 in the neutral crosscut. The area had previously shown signs of deterioration and had been cribbed out to prevent entry, and provide supplemental report.
At approx. 8:10 AM a roof fall above the anchorage was found in the old #4 room and extending to the #2 room of the 4 rt. off 4 North Section. The fall measured approx. 150' long by 18' wide and 7' high. The fall started at 1+00 in the old rooms.
A roof fall occurred above the anchorage point in the 5B longwall section in the tailgate entry at survey station 86 + 55. The dimensions are 15' l. and 10' h, the width of the entry. The fall has been mined past.
We experienced a total power outage from provider (Avista) to all U.S. Silver properties. There were no personnel in conveyances. #3 cagers were in pocket. All underground workers were instructed to stand down until power came back up. The hoistman notified Avista and was told outage weather related. Power came back up at 4:01 and hoist and fan start ups went well.
A roof fall was discovered in the #9 seam in the old 4A panel #7 entry xc-4 at spad #2+80. The approximate size of the fall was length-20ft,width-20ft and height-8ft.
At 12:35p.m. a fire was reported on the 34014 Oldenburg Drill hdg. in the 95-17 Hdg. in the 85-15 stope.Foreman called for an evac. of the mine. MSHA,mine rescue,911,G.M. was contacted at 12:50 p.m. At 1:35p.m. all accounted for except 3 util. EE's.They could not evac. w/o passing thru smoke, they comm. to the surf. by phone, rescue arrived and trans.men to the srf.at 3:50 p.m.
None injury. A pillar fall occurred and water came in later. It was discovered on pre-shift for the night shift.
IGNITION - Occurred in the headgate entry while burning bolts off the crusher element, which burned for 15 seconds and was put out by water and rockdust.
Roof fall occurred in the No 1 return entry 270 ft outby the coal face. The fall was 20 ft long 30 ft wide and 7 ft thick. The fall will not be cleaned up. The entrance to fall is dangered off and supported as per roof control plan.
The Coy Hoist lost power, the General Mine Foreman was notified and evacuation began. The electrical department was called in to restore power while the evacuation was in process at the Hicks Shaft, the power was restored at 5:01 P.M. evacuation was stopped and work was resumed.
Stresses related to two separate faults along with unconsolidated slickenside shale roof
At approximately 2:50 pm the service elevator was removed from service due to arcing in one of the motor brushes. All motor brushes was replaced and the elevator was put back in service.
A roof fall was discovered in the #1 entry, second intersection outby the 3 Right Mains faces, Spad 3558. The fall was approximately 30 feet long x 20 feet wide x 6-8 feet high.
A roof fall was discovered by Mine Foreman on old 2 F Panel 1 Block inby Spad 9222. There was no interruption in ventilation and no equipment involved.
#3 Hoist tripped on UHM watch dog.
A hydraulic hose failed on the slope hoist car making the hoist car inoperable for more than 30 minutes. The hydraulic was replaced.
AN UNPLANNED ROOF FALL WAS DISCOVERED IN THE #1 SOUTH MAINS AT BREAK #31 IN THE #2 ENTRY. THE FALL MEASURES APPROXIMATELY 100'L X 19'W X 9'T. THE ROOF FALL DOES NOT AFFECT VENTILATION OR TRAVEL.
During inspection a broken bolt was found. Emergency Escape Facility was removed from service until a proper bolt could be found and replaced. No injuries occurred.
At approximately 1:00am the service hoist went down on fault because of the drum breaks. The drum breaks were adjusted and the service hoist was put back in service at approximately 6:00am on 12/21/2011.
The Koepe resync motor burned up causing the conveyance to be down for more than 30 minutes. Mine evacuated via production shaft. No injuries or other problems. Koepe resync motor replaced and Koepe put back into service with no issues.
Roof fall on #8 belt 33 blk 6-7 crosscut area 15'w x 40'l x 9'h.
Airway examiner found fall on his weekly run. There was no equipment involved and no interruption in ventilation.
An unintentional roof fall occurred on the 5B belt line at 106 crosscut it measured 20'x20'x15' high. The fall did not affect ventilation, cause personal injury or affect passage. It will not be cleaned up.
While cutting holes in two plugs inside of two fine tails tanks, sparks contacted a plastic pipe and started a fire. Duration 1 hour 37 minutes.
Hoist motor failed due to loose conductor and motor stator nut. Mine was evacuated at 7:57a.m. and completed at 8:21 a.m. Repairs were made, hoist tested, and miners returned underground at 9:15a.m.
A fall was discovered on the A-8 Panel at #15 Block (Between #4 and #5 Heading). The area was bolted with 6' fully-grouted resin bolts.
A rock fall was discovered on 2 East Mains in the #2 entry at break #3. Fall measured approximately 19' wide x 30' long x 6' thick.
A rock fall was discovered on 1 left off 1 South East Mains in the #7 entry crosscut 28. Fall measured approximately 20' wide x 20' long x 6' thick.
An Unintentional roof fall occurred at 8:15pm on the #2 unit, xc #8, and in entry #7.
A roof fall occurred in the #2 entry of the 1st Main North off the Second West from 25' inby XC 18+24 to 10' outby XC 20+64. There were no injuries or disruption to ventilation. The fall was approximately 90' long by 18' wide by high. The fall was discovered around AM. There was no equipment involved.
A 30-foot long x 18-20-foot wide x approximately 6-foot high roof fall occurred in the # 5 entry of 7-Butt Section at the face and last intersection. The area was supported with both 4-foot and 6-foot fully-grouted resin bolts. The section was experiencing roof water. The cave will not be cleaned up. No equipment or ventilation affected.
The main shaft hoist power kicked out after the second man trip was 20' below surface level. The electricians and programmers found that 3 electrical contacts were bad in 2 emergency stop switches. Once replaced, the hoist was fully operational.
Unplanned roof fall occurred at #10 c.c. on the #3 belt the c.c. to the right at spad #241 between entries #2 and #3 it is approx. 40' long, 8 to 10' high and 8' - 10' thick. No injuries or entrapment involved. However it did fall on a diesel forklift. No ventilation or travel way involved.
The #1 and #2 2 meg electrical generators had caught fire and burnt beyond repair.
THE PRODUCTION HOIST WENT DOWN ON 12/14/2011. A MINE EVACUATION WAS STARTED AT 12:09 P.M. BECAUSE THE PRODUCTION SHAFT IS THE SECOND WAY OUT OF THE MINE. IT WAS DISCOVERED THAT THERE WAS A BLOWN FUSE ON THE NUMBER 0NE DRIVE FOR THE HOIST. THE FUSE WAS REPLACED AND THE HOIST BECAME OPERATIONAL AGAIN AT 12:31 P.M.
An unintentional roof fall occurred on the 1 North 3, between Entries 5-6 Crosscut 17. The roof fall measured approximately 40' in length, 17'-18' in width, and 6' in thickness. A 103 (k) Order (8437763), was issued December 13, 2011, by MSHA Inspector.
At 11:30PM, Holcim EE's noticed flames at Geocycle, LLC (ID# R216) alternative fuels shed. Fire department contained fire by 12:15AM. Fire out at 2:38AM when FD left site. Roof of shed was damaged beyond salvage. Building is being stabilized to allow removal of remaining material. RCFA initiated. AFR policies and procedures will be updated to prevent occurrence.
A non-injury roof fall was discovered on the #4 East Mains bleeder one crosscut inby survey station #3156, and two crosscuts outby survey station #3153, in the #3 entry. The roof fall is approximately 80' in length X 18' wide and ranges in thickness 4' to 6'.
A roof fall was reported around 10:10 a.m. in the #3 entry of P-Butt Belt Line at break 27-38. The fall is approx. 85' long, 40""-48"" high, 20' wide.
Damage occurred to the A Shaft man hoist resulting in the hoist being out of service for a period of time longer than 30 minutes. The north gate of the hoist was damaged and was being struck as the elevator ascended.
An unintentional roof fall was discovered at approximately 10:30 p.m. on the 3 East Mains at Crosscut No.52 in the No. 2 bdg. The fall was in the intersection. The fall was approximately 6'-8' high and 20' wide and approximately 10' long. All approaches to the fall were timbered off. The fall did not interupt ventilation.
4701 #1 Hoist had computer control malfunction. Electrician's had to re-boot the computer system to get Hoist controls functional again.
A call was made to the MSHA Hot-Line stating that a roof fall had occurred on the continuous miner on the MMU No. 001-0 Section. This call was made in error. The report is to comply with the part 50 reporting of accidents.
The mine fan became inoperational at approximately 8:20am due to transformer issues. All employees were withdrawn to the surface immediately. Mine fan was put back into service on 12/10/11 at approximately 5:40pm.
A roof fall occurred in the travelway (#5 entry) 140 feet inby survey station 156. The fall was approximately 40 feet long x 18 feet wide x 4 feet high.
A roof fall was reported on 12/01/2011. After further investigation of dangered off areas 5 falls were reported. All five falls had been dangered off with a physical barrier and were assumed to be inactive areas.
An unplanned roof fall occurred in the mine near spad # 1374, that measured 40-50' long X 20' wide X 5-6' high. No injuries were reported. The area was developed on 12/15/09.
Roof fall at crosscut #10, #3 entry of the 6th Right Section. Approximate dimensions are 16 foot by 12 feet wide and 8 to 9 feet high.
A small methane ignition occurred in the face of the #5 entry of the 2nd West Mains when a 5-foot mechanically anchored roof bolt was installed in rock at the top of the rib and methane in a roof crack was ignited by friction or sparks.
The weekly examiner was traveling the return with an MSHA inspector and they found a roof fall in #2 entry of Cow Branch Mains near Spad # 4003. The area was dangered off and cribs built in the area.
Found a roof fall during air course run it was in 1st SW Mains #7 intake roof it measured approx 50' long & 18' wide 8'-10' high.
#1 entry on #1 belt between #14 and #15 cross cut on the left inby rib. 30' long x 8' wide x 3' high. 1 bolt was broke off, approx. 8 bolts exposed and 1 bolt out. The bolts are 42"" fully grouted.
Plant air pressure was reduced below 80 psi causing the emergency air damper to open on K1. This led to a kiln shut down from pulling high ID fan amps. An unplanned ignition of fuel occurred in the fuel system during 1.5 hours post solid fuel addition.
Fall was found on new haven mains, 3 breaks inby spad #1602 Fall was 18'W x 20' Long and 4' Thick. Roof support was 4' glue bolts.
Shift supervisor reported power pole at the K11 substation was on fire from electrical arcing, presumed to be due to the accumulation of snow. 12:51 shift electrician cut power to substation. Once the arching subsided flames went out but pole continued to smolder past 30 minute time frame. Smoldering extinguished at 2:05. At approx. 1:10 MSHA was notified.
An unplanned roof fall occurred along the #1 belt at the #6 crosscut.
An unplanned roof fall occurred at break #51 on the #6 belt line in the #5 entry. Approximately 19,200' from the active face.
After investigation it was determined that a roof fall occurred on the mainline track in a crosscut at 336+00. The actual time of the fall is undetermined. The fall was 26 feet wide 15 feet high and the length was undetermined due to access.
The brake system on the hoist car at the mine malfunctioned for a period of time that exceeded 30 minutes. When examined it was found the 2 wires had grounded together. No injuries were reported.
An unintentional roof fall was found in our company No. P5-1 Panel, x-cut #7, entry #23, during a weekly examination. The fall did not affect passage of miners, affect ventilation or cause harm to miners.
Pressure build-up and release from within the cooler. Sheeting on the south wall of area 500 building was pushed free of screws where sheets were secured together. NOTE: THIS DOES NOT CONSTITUTE AN ADMISSION THAT AN IGNITION OCCURRED. THIS REPORT IS FILED OUT OF AN ABUNDANCE OF PRECAUTION. A CONTRACT EMPLOYEE WAS INJURED; A 7000-1 WAS FILED BY THE CONTRACT COMPANY.
A non-injury roof fall occurred at this mine approximately 1 break inby spad # 4161. The airway or travelway was not impeded. Area was breakered off and supplemental support was installed.
A roof fall measuring 20' long, 20' wide and 9' thick was discovered inby spad 3147 # 2 entry in # 8 Mains. The fall was 8100' outby the working section, ventilation, equipment, and the travel of personnel was not affected. No injuries occurred from the fall, the type of roof support in the area was 48"" resin bolts.
Elevator stalled for 211 minutes. MSHA was called on 12/05/2011 @ 8:38 a.m.
Elevator entrapment - Two contractor employees were entrapped for 2 hours and 45 minutes
Plant air pressure was reduced below 80 psi causing the emergency air damper to open on K1. This led to a kiln shut down from pulling high ID fan amps. An unplanned ignition of fuel occurred in the fuel system during relight.
A roof fall occurred in the #4 entry of 1st North West Main intake travelway. Fall begins approximately 50ft outby SS #5836 amd extends approximately 60 feet outby and is the entire width of entry.
An unplanned roof fall above the anchorage zone occurred on the MMU 006/007 (#2 section) belt line between spad number 1689 and 1717 in the belt #4 entry. the fall was approximately 75 feet wide X 18 feet wide X 8 feet thick. No injuries or entrapment were involved. No escapeway or ventilation involved. Conveyor belt line was under the fall.
The elevator at #6 portal was inoperable when a locking mechanism (bar) malfunctioned. The elevator was down from 2:35am until 9:35am.
Unintended roof fall was discovered at approx 2:35 am on the 3 east mains at crosscut No. 11 in No.4 hdg. Fall was in intersection. Fall was approx 10'high and 20'wide and approx 30'long. Fall caught high voltage cable and cable had to be recruited around the fall. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up.
The emergency escape hoist became inoperable during an inspection. The PLC program malfunctioned and the hoist would not operate. The PLC was reprogrammed and the hoist returned to operation at 5:15 PM.
Compliance personnel were inspecting the mine for compliance issues and located the fall during their travels.
While doing a daily hoist inspection, employee found a broken guide rope on the Gallitan County North Fan emergency hoist. The rope was replaced.
While traveling in the mine a roof fall was found in the croc cut # 20 between #3 and # 4 entries in the 2nd Right section. This fall is a continuation of an area that had been cribbed and dangered off and later fell. The new area of the fall is 20'W x 20' L x 6' H and has fallen over a return/intake stopping and a single row of timbers.
An unintentional roof fall occurred at the New Future Mine 1W Tailgate (side bleeders) entry #3, crosscut #2 @ 900W. The fall was approximately 25 feet in length, approximately 20 feet in width and approximately 12 feet thick. The fall did not affect ventilation or passage of people. The fall will not be cleaned up.
At 8:35 pm the kiln operator received an alarm that the #1 coal mill system vents opened. The kiln and coal mill system were shutdown. During the investigation it was discovered that the coal mill inlet duct was hot (surface steel). An accumulation of coal dust had accumulated at the inlet duct and began to smolder causing an ignition source in the system.
Event was called out as a roof fall at south 25c/c,3-4 entry BUT was actually 22c/c,3-4 entry. This was ONLY a continuation of a previously reported fall. The area had already been cribbed off AND dangered off and reported to the proper agencies. No injuries or fatalities are associated and no ventilation or escapeways are interrupted. No inundations are observed or detected.
The Slope hoist rope was damaged and taken out of service for more than 30 minutes. This prevented equipment from being hoisted out of the mine. It had a kink in the wire hoist rope and needed taken out of service to re-teminate the wire rope. How the kink occurred is not known. The strands were not damaged and no injuries or entrapment as a result of the kink in the rope.
For unknown reasons and or witnesses the victim fell into the jaw opening and was subsequently crushed.
Employee was placing removable legs on a transfer conveyor. Conveyor was held in place by a loader. Removable leg fell and struck employee in the face, fatally injuring the employee.
Excavator was working in Redwine pit, highwall collapsed causing rock to fall on excavator. This caused a fatal injury to operator.
EE was shoving top soil away from the highwall. He backed up over and through an 8 foot berm that had been constructed at the edge of the highwall. The dozer went over the wall rolling over and throwing him out of the cab.
Mechanic was working on a schwing concrete pump which had been sticking and not functioning properly. The mechanic noticed a piece of concrete in the discharge tube and reached into the tube to pull out the concrete. At that moment the pump completed a cycle with the mechanic's hand in its path. The pump edge amputated four fingers and part of the mechanic's left thumb.
Scoop changing cables were stuck in mud and employee was pulling and jerking on them when he experienced a pain in lower abdomen.
Employee was assisting with loading of Drill Steel onto the bed of the Boom Truck. The forklift operator set the Drill Steel into the cradle and backed away. Employee stepped forward to begi n tie down of Drill Steel when the Drill Steel rolled off and struck the employee in the lower left leg.
Employee was installing roof bolt and reached up to straighten plate. He accidentally hit the up pressure and his left index finger was caught between roof & roof bolt plate.
While installing a roof bolt, a piece of rock fell striking his right hand and injuring his finger.
Employee was placing a guard on the back of a flatbed truck when he pinched his finger between the guard and truck, amputating the tip of his left long finger.
Employee was dumping a muck bucket and caught finger in locking ear.
While changing out the long hole steel Miner grabbed a-hold of the steel while driller was rotating to remove it. The Steel became bound up at an angle and pinched his thumb between the steel and the guide slide.
Employee was exiting down a boarding ladder, from work plaform and felt the knee pop.
The man was walking down the entry when he struck his head against a ledge in the roof causing pain in his neck.
Plant mechanic was working to clear a rock lodged in the tail pulley of a conveyor. Equipment was off and locked out and tagged out. Using a breaker bar to pry loose the rock, the miner lost his grip on the bar and fell backward but his left foot remained planted as his body twisted. This resulted in the dislocation of his left ankle and broken left foot.
Employee was attempting to remove a man door from a pallet of block, in doing so while sliding the door off a stray block fell off striking employee on the foot.
EE was sitting in CAT 30, and was being loaded by loader operator. Loader operator was easing a large rock into the back of CAT 30. The rock dislodged from the bucket and created a concussio n in back of truck, jarring EE.
While lifting miner cable experienced back pain that extended down his legs. Employee continued working until 01-03-2012.
He was changing out drive shaft on vessel and was going back in with new shaft while still attached to chain hoist he stopped hoist to rest, placed his hand on shaft, weight shifted causing shaft to move and pinch his finger in between chain, shaft and vessel.
Employee was stacking 100 lb. bags of sand and twisted and wrenched his lower back.
Employee was installing roof bolts and as he was removing the drill steel from the bolt hole the drill steel struck his right middle finger.
Stated: Employee was walking between road grader and rib to access cab of grader. Waterline was hung at 5' off mine floor at this location. Employee hit head on waterline coupling, slipped and rolled her ankle. Later diagnosed with fracture.
Employee was cutting crib blocks. Slipped on muddy slope floor. Causing lower back strain
Employee was moving a portable heater from the East side of Packhouse #4 to the West side where he would be working. As he wheeled the heater out of tunnel, the wheel got caught on the tap box and his shoulder jerked back, causing a great deal of discomfort. He then reported to supervisor.
The worker was cutting a piece of stainless steel plate with a metabo grinder. The grinder became bound up and kicked back, cutting the workers thumb.
Fueling D11R Dozer when he slipped, the employee caught himself and strained his shoulder.
EE stated that while carrying a bag of rock dust along the 1 South #2 Belt at 82 Block he slipped and twisted his right ankle. The EE returned to work his next regularly sch. shift. He began to miss work on 1/9/12.
We are unsure when this started EE went to her doctor for painful back she had been having for a while and the doctor required her to be off work until 2/21/12 so she could go to a specialist for further testing of what is happening with her back.
Employee along with another employee was carrying a piece of angle iron across the road in front of the plant office. He stated his right knee ""popped"" and started hurting. He said he could not put his weight on it. Employee was carrying 72 lbs. Employee was transported to Primary Care Center in Hazard, Ky where he was seen and released to return to work.
The employee was involved in push-pull scraper operation. He was in 2nd scraper and hooked to 1st scraper with the bale. The 1st scraper moved forward causing the 2nd scraper to unhook which caused it to drop down hard, causing a trauma of compression to the center of lower back.
Employee was using a lifting jack to raise and level a belt take up. A slate bar was being used to lift take up. As he was using the jack the employee caught his finger between the jack and belt structure over head resulting in a fracture.
Washed truck, was getting in truck and slipped grabbing door with right arm.
Employee was carrying two buckets to the lab. He tripped on a slightly-elevated section of asphalt. It was not slippery or rocky. He was unable to break his fall and fell hard on his right knee. He felt a sharp pain and was unable to support his weight on it. He was driven to Urgent Care where they identified internal fluid buildup and a potential bone fracture.
Employee states that she was closing the cab door on a drill as she exited when her left thumb was pinched between the door latch and the latch striker mechanism. The thumb sustained fractures, lacerations and an avulsed nail.
EE EXPERIENCED PAIN IN HIS LOWER RIGHT ABDOMEN ON 12-28-2011. INITIAL DR VISIT ON 12/29/11 REVEALED HERNIA. EE SOUGHT 2ND OPINION. THAT DR DIAGNOSED ONLY A MUSCLE PULL/STRAIN AND GAVE EE A NO RESTRICTION DR NOTE. EE BELIEVES THE CAUSE WAS HEAVY LIFTING AND SHOVELING OF SALT OVER EXTENDING PERIOD OF TIME. RELATED TO ACCIDENT REPORTED JUNE 2011. EE QUIT 1/13/12.
While flushing the Rev 3.1 with water, he got a cramp in his back, he kneeled down and while kneeling on the metal grating he rolled his right knee over the grating and his knee became very sore.
EE was walking through Mill building; she ducked under new Tails Line pipe and upon standing up she hit her head on the pipe. EE lost her footing and stumbled forward, striking her head on the contractor gang box and then falling and landing on her right leg and arm.
Employee was using a slide hammer and got finger pinched between surfaces causing a laceration.
Operator was backing up the dozer when he hit a rock with one track. Said his neck was stiff and went to hospital the next day. Said he had a pulled muscle.
Employee was removing boxes from pickup truck, experienced pain in back area
While using a hammer, a piece of metal hit him in his right eye.
Ee was on his knees changing a bottom roller. When he reached for the roller his back ""gave out"". He is still off work as of 1-12-12 and in physical therapy.
Employee was carrying his personal tool box (2'x10""x10""), he reached out to set the toolbox down and felt a pain in his lower back.
Employee was getting in jon boat, was walking to the rear of the boat to operate the boat motor, employee tried to step on the metal seat to get to the back of boat, when he stepped on the metal seat he slipped, hitting the left side of his torso on the seat/bottom of the boat.
Dip in the haulroad at the South Mill standpipe intersection.
As EE was stepping out of the man van his left knee popped. EE was taken to the ER where he was taken off work.
The employee was repairing the equipment when he was trying to loose the industrial screw (thirty pound screw) he was distracted and the screw slipped on his finger and hurt him.
Employee was roof bolting when a piece of rock struck him on the shoulder. The rock was 12"" wide 36"" long and 2"" thick
Employee was walking by miner when a rock slid off the miner striking him on his left foot. The rock measured 30"" x 46"" x 0"" - 10"" thick. The incident resulted in a fracture.
While lifting a bearing to install a plow bolt in a take up pulley the bar slipped causing the bearing to come down on the employee's left thumb and index finger.
Employee was lifting a ramp onto a tow/haul trailer when he felt a pull in his lower back causing pain that dropped him to his knees. Seen at the ER, originally released to full duty, put off work effective 12/29/2011 and sent to physical therapy on follow-up 12/28/2011.
Employee stated that he felt faint. He stated the last thing he remembered was squatting down.
Employee was operating rock truck going up a hill. The truck spun out and slid backwards straining his back. Employee did not report it to his supervisor until 12/27/11 and went off work on 1/2/2012.
Employee was using a lift jack working on a gravel car and strained his back. Employee worked until 1/20/12 then went off work.
EE was getting into the skid loader. Left foot was on the platform. Right foot was on the top of the blade. Due to muddy boots, right foot slipped off of the blade. Right foot went between the machine and the blade. He landed on his right side on the blade of the skid loader.
Walking surface was covered with frost when employee attempted to jump across lined ditch. Suffered injury to right ankle.
Employee was struck on top of the right foot while using a 3 ton tugit to lift a coul-arm over the handrail. The hook-up slipped causing the coul-arm to strike his foot.
While lifting screens into a truck the employee felt something pull in his lower abdomen.
Employee was supporting miner cable on his right shoulder when he states he felt a sharp pain.
Employee lifted 50 lb salt bags to put down on icy sidewalks.
Injured was standing beside the roof bolter when a rock fell from between existing roof bolts and struck him on the head and neck area.
EE was walking out to diesel tank when he slipped on ice and may have done internal damage to right knee by twisting it. Diagnostic testing will be done on January 5th, 2012 by an orthopedist.

After investigation completed on 1/25/2012, it has been determined that an employee walking on the King 5320 stockpile felt a sharp pain in his left calf.
Employee was watching water line being pulled, when the water line broke striking the employee.
The driver slipped on clay/mud while trying to hook up his harness. He fell flat on his back and injured his shoulder. Dr.s are calling it a cervical strain.
EE was unloading lime from trailers. While doing so he had lime enter the top of his gloves. The reaction of sweat and lime caused burns to his wrists. He reported the incident by phone late in the afternoon. He described the injuries as minor and denied need for medical attention. The next morning his left wrist was red and appeared infected. I sent him to get medical attention.
Using wrench, wrench slipped off and employee hit right hand on frame of machine.
Laborer (ee) and loader operator were cleaning material from under the #1 feed conveyor. EE placed his hand between the conveyor frame and loader bucket. When another ee moved loader ee right middle finger was caught in between.
He was operating a supply motor on the track and it derailed and caused him to hit the mine roof with his face and shoulder. Diagnosed with cuts and contusions. He is working at a restricted duty status.
EE stepped out of dozer onto a rock and twisted his left ankle. He did not notify us until 12/22/11
While doing maintenance on conveyor #2123, slipped in mud causing contact with pipe support for conveyor.
Employee was welding when the welding rod stuck, he then pulled loose and struck him in the right eye, emergency care was given on section, the employee had welding slag and flux embedded in his eye. Was seen at the ER for treatment.
The 2nd shift production crew was exiting the mine on a Mac 8 rubber tired mantrip near break 18 when a slate bar that was located on the mantrip caught the mine floor resulting in the bar being impaled into the upper leg of an ee that was seated on the left side of the man trip. The ee was taken to the hospital were surgery was performed to remove the slate bar.
Employee was setting bits on miner when he turned his head and felt a sharp pain that went down his left shoulder causing his left arm to go numb. He returned to his regular duties until 2/3/2012 when he had surgery on his shoulder.
Employee was installing a clutch assemble and while holding it in his hand it slipped out and fell on his right hand.

EE was assisting fellow EE in moving the anchor for #225 shuttle car. The operator of the shuttle car turned on the shuttle car to reel up the cable to move the anchor. While the anchor was being reeled up it struck a rock in the roadway causing it to turn suddenly and strike the injured EE's right knee.
Employee states he was feeling pain in the lower groin area. There was no specific accident or injury, but since he's a material handler he felt the problem was from lifting, moving, and carrying supplies over a period of time. He was referred for an evaluation and was diagnosed with a hernia which was determined to meet WC guidelines and accepted as an occupational illness.

The employee stated that the service maintenance ride was down, so he was walking with a can of hydraulic oil to take to a coal hauler to fill it up, when he stepped in a hole and twisted his knee. The IE continued to work, then on 1/20/12 had surgery to repair a torn meniscus.
Brace plate caught and kicked out causing the brace to roll striking EE's right foot glancing off his steel toe boot and braking his right big toe on 12/18/11 at 16:30. EE was taken by truck to seek medical treatment, x-ray confirmed broken toe on right foot.

EE was walking down the steps in the wash plant and slipped. He tried to catch himself on the hand rail on the steps. He injured his right shoulder.
Employee had parked his truck in the truck parking lot. Still inside the truck cab, co-worker was backing his truck in. Did not see EE's Truck. EE thought the truck was going to hit his truck. He jumped from his truck, hitting ground fracturing his left wrist.
Setting up to use jackleg drill; while holding drill with left hand miner had right hand on leg of drill, head came down on right hand striking top of right hand and two middle fingers. Miner flew out of camp on December 19th, was seen and sent for evaluation - x-rays taken on Thursday December 22nd indicated fractures to wrist and finger.
Employee was in the #7 entry, standing behind the miner boom, when a piece of rock (approximately 2 1/2' X 3' X 2 1/2"") slid off the shuttle car striking his left foot resulting in a fracture.
Employee was crossing belt when he slipped and fell on the belt. Employee was transported on moving belt 1500' when he stopped the belt. Received a fracture to the L2 lumbar vertebra. Mine was idle from 12/22/11- 01/03/12
Miner was in cab of Ford fuel truck assisting mechanic by ""bumping"" engine when necessary. Miner was wearing Tyvec coverall suit. Miner stepped out of truck and foot slipped on top step causing Tyvec pant leg to get caught on step. This flipped miner out of cab, landing on floor, resulting in two lacerations to scalp and a broken rib.
Company # 808 scoop was being trammed though a curtain which had been rolled up on 1 South Mains. The curtain became caught on one of the headlights of the scoop. A fellow EE raised the curtain off the scoop. The scoop operator proceeded to tram the scoop and the injured EE was struck on his right foot by the tire of the scoop.
The man was installing 72 inch roof bolts. When the worker was bending the roof bolt, to push the bolt up into the drill hole, he felt a sharp pain in his right shoulder area.
Worker was attached to a 30' tall rebar cage @ the 10'-15' (foot) height. For an undetermined reason the cage collapsed, bringing worker down with it. A rebar struck him over the left eye and across the ribs. The blow resulted in swelling over the left eye & soreness of the chest & ribs. Required medical attention & several days off to recover.
The injured was riding a rubber tired mantrip on his way to his work site when the mantrip dropped into a hole in the roadway on 4 South. The mantrip bounced causing the injured to bounce up and down in his seat dislocating the tip of his tail bone.
Employee was walking by the Power Center when a piece of draw rock fell striking his right leg.
Employee was jacking rail car onto rails when handle of jack slipped out of hands and struck him in chest. Started to lose time 12-30-11.
EE was unloading heal to put into trash bucket. The heal usually is 1""-3"" thick. The EE was trying to break heal when 2-3.5"" strips fell over on his right hand and thumb. Dr's note soft tissue bruise.
Employee was riding on passenger side of a Damascus permissible 2-man ride when the ride malfunctioned causing the ride to abruptly turn into the belt line. This jolted the employee into the control panel in the center of the ride. This resulted in a few fractured ribs the employee was kept for observation and released and remains off.

Injured employee was walking down the track entry when he stepped in a hole and twisted his right ankle. He was transported to the ER and diagnosed with a right distal tibula fracture. He was excused from work and referred to Orthopedics.
Picking up rock off prepping car for highwall miner and co-worker dropped pin in tray when it bounced and hit right thumb and hand braking the right thumb just about the joint.
Doing mechanic work on loader bucket. Lifting plate onto bottom of bucket of loader with other mechanics help. Pulled his back, pain occurred immediately.
Employee was bolting an open break between #6 and #7 entry. Employee went to bend a bolt and felt a sharp pain in his lower back and tail bone. Employee stated that the pain went down to his left leg.
Employee was injured 12/15/11 when he fell off motor grader. He has been on medication to relieve the pain but has not found relief. His first shift missed was 3/17/12. On 4/9/12 after completion of a workers compensation investigation, Westmoreland Resources was informed that a herniated disc in employees back was the result of the on the job injury.
IE was assisting maint crew install latch bar and lever on shovel. Once the latch bar slid into the trip lever the IE stumbled backwards, IE tried to regain balance and his arm got bound up under the trip lever bending his wrist backward and resulting in a fractured left arm just above the wrist. The fracture was set at the ER.

Employee was operating a diesel locomotive and had gotten off the motor to throw the track switch. When he did so he felt a pain in his right shoulder and upper chest. Employee continued to work and finished his shift. After his shift the pain increased and he sought medical attention.

Employee did not report until 12/27/2011 - While assisting lifting covers of RRII roof bolter he thinks he hurt his lower back. It did not start bothering him until 12/25/2011. He has had previous back injuries with previous employers.
Employee was building a crib for roof support using 30-inch crib blocks and was using a pick hammer to pull cribs towards him. When doing so the pick hammer glanced off one of the cribs and struck the employee in the lower leg causing injury.
On 12/14/11, the employee stated he was tramming the 550 scoop in the inby direction heading towards the #4 face. He stated he went through a set of fly pads and ran the scoop into the rib causing the scoop to jar and striking his right shoulder and elbow inside of the deck. The incident was deemed a lost time reportable on 1/11/12 for a strain to the shoulder and neck.
While using a four way wrench to loosen a tire nut on his Johnson Mantrip, the EE felt a pain in his upper left arm which resulted in a torn bicep.

EE injured was working in the area around a D11R Dozer. He stepped on a rock that was covered with mud. On the next shift he reported incident to superintendent and denied medical treatment offered. That evening the EE visited a Dr. for evaluation. The EE injured deemed to have a level 2 sprained/strained ankle. Investigation pending by company.
The employee hyper extended his left wrist operating the RBM standing at the inch controls. The machine dropped suddenly in soft and busted up bottom causing the control lever guard to strike the employee's wrist.
Employee was pulling fuel hose and nozzle off rear reel of XSV2 service truck attempting to fuel WA600-6 Komatsu loader. While pulling fuel hose employee felt a pain and soreness in his right shoulder.
While in the process of dismounting supply motor employee felt pain in his left knee. He stated that he has a history of knee problems. On 12-23-11 an MRI was performed. On 1-6-12 he was seen by a specialist and was informed it was a new tear. This report was delayed to determine if this was a new or old injury.
Filing only to comply w/MSHA regulations; however, company's investigation doesn't support employee's report that his shoulder tears occurred at work. Company was officially notified 2/22/12 that Wk Comp agreed to pay claim under a disputed liability. EE notified Company on 1/13/12 that approx.5 weeks earlier he was injured & had been doctoring. We are contesting this filing.
An employees haul truck rolled backwards making contact with a highwall at an angle that caused the truck to tip over onto its right side.
EE attempted to climb into the bed of his sterling dump truck when he lost his balance and fell to the ground. EE suffered a broken left arm (upper). EE was transported to Mt. Carmel west hospital.
The ee was positioned in the operator's compartment with the driver of manbus. The bus traveled near break #36 the mine height changed from 72"" to 54"" quickly resulting in the belt man's head contacting the mine roof due to not being positioned low enough to provide vertical clearance. The belt man received an abrasion to the forehead.
Injured worker was pulling on a large wrench trying to remove a nut. The wrench slipped off of the nut throwing the worker into a metal support causing the injury.
Employee was cutting a plate with a track torch when plate fell on foot causing a laceration.
Employee was in an area where the operator working the control box for beams could not see him. Employee was pinned between base frame brace and a beam that was being moved; was struck in the lower abdomen.

EE was backing the miner out of the cut of coal when the cable went under the miner. He went up to remove cable from under the miner and accidentally bumped the remote control box against the cable stand-off, activating the tram lever and then the panic bar. He was caught between the coal rib and the stand-off of the machine.
The door latch was stuck. He pulled the latch and strained his arm.
Employee twisted his left ankle when he was unloading brattice blocks and his left foot got tangled up in a rope.
Check top and rib before taking coal section. EE noticed rock coal shell chunk started coming off rib and EE pushed IE out of the way. Chunk of coal hit IE first, then a piece of coal fell on EE's ankle.
Employee was operating a dozer and smelled diesel fumes in the cab. He began getting sick, upset stomach, stomach pain, chest pain and coughing. He was seen at the ER, diagnosed with bronchitis that was exacerbated by inhaling diesel fumes. He was admitted to the hospital for observation for 2 nights. Released on 12/15 to follow up for release to work on 12/19.
EE was tied off with lanyard as he was on belt preparing to rig the pingate. Client's didn't see lockout causing belt to release and EE to lose footing and fall into pingate.
Employee was lifting jackhammer out of a low spot to change leg when he slipped on a rock & injured back.

Employee was installing T-channel in #2 heading. The channel was laying on ATRS and while he was putting glue into bolt hole, the channel fell striking his finger against the drill pot causing a laceration that required stitches. Started missing work on 12-14-11.
EE was aligning torque head onto nut. EE instructed torque machine pump operator to bump torque head to align onto nut. Pump operator interpreted that as to start torquing the scoop bolt. EE had slid hand under torque head and in between scoop flange. EE received laceration to hand.
12/9/11 EE was helping to hang bolting machine cable. The cable reel hung up and jerked cable down catching EE from behind. The EE cont'd. to work and did not miss any work until 3/22/12. He had O.P. surgery on his knee on 3/22/12. This was his first day away from work due to this accident.
Employee was removing rock from rock crusher when he pulled muscle in back.

Miner was packing and stacking 100 lb. bags of barite. Instead of dropping the bag in place on to the pallet as he was trained to do, he threw the bag and strained his back
Employee was walking down the stairs out side of the con spec building and as she went to step off from the last step of the stairs, onto the ground, her foot slipped and she stained her right hip as she struggled to regain her balance.

He was diagnosed with a fractured left little finger and right knee cap.

Employee was lapping belt on a mine sled on the surface, he was pulling on a loop of belt to straighten the laps and felt pain in the left knee.
Employee was working on the suction on his roof bolter. He asked his partner to assist him. He was reaching under the boom area and had his hand laying on the long arm of the boom and asked his partner to raise the boom. As he did so, his hand was caught in the scissor pinch point causing a crushing injury.
EE stated he was driving outby when he was exposed to smoke from friction on a nearby belt. This 7000-1 form is being submitted in reference to citation No. 8142583.
Moving a pump because it was out of water did not want the pump to seize as employee moved it over out of #1 he slipped & fell. Employee felt a slight pull he ignored it because it didn't bother him until he got home around 8:30-9:00am.
Co-worker was operating a skid-steer loader doing clean up work, when parking the skid-steer loader he struck an implement and pushed it into employee spraining employee's ankle.
Contractor was attempting to clean windshield on his coal truck. Contractor had his left foot on drivers side tire and right foot on driver side steps. Contractor slipped and fell backward onto another truck parked beside him. Total fall was approximately 4 feet. Slip, fall, strain.
Using a hammer and punch to knock pin out of boom of miner, a piece of metal either came off of hammer or punch, becoming lodged in the right side of neck. Employee was seen at ER, then adivsed to have it surgically removed. Employee returned to work the next shift, metal was removed on 1-9-12, returned to work on 1-12-12.
Mechanic was performing maintenance on John Deere 850c Dozer when he slipped and fell from the tracks to the ground injuring left leg.
EE was hanging a fly pad and was nailing it using a hammer when he hit his left thumb.

When he reached to pull the spring out, the Harrington 3T chain hoist gave way, allowing the screen to fall down mashing his four fingers between the screen pedestal and the spring. He received a contusion only. He is off work until further notice for therapy.
Employee was adding tension to the conveyor chain on the unit feeder. The injured employee was positioned at the tail shaft of the feeder when the tail shaft cover broke free from the feeder frame, striking him in the face. He started missing time on 12/08/2011.
Employee was getting on the plant elevator when his right foot slipped, causing him to lose his balance. He twisted his left knee which caused him to buckle and fall to the ground.

Employee was trying to fix plastic guarding around a drive at a junction. He had just changed his knife blade and was attempting to cut a zip tie while he was holding the guard. The knife cut through the zip tie and created an incision in his left wrist which required stitches.

EE slipped and fell while fueling crushing equipment. EE has a sprained lumbar-sacral sprain with radiculitis and a sprained rotator cuff.

Independent contractor was on site constructing a covered fuel deposit. The final stages of the project were being completed when the carpenter lost his balance while removing some temp. braces and fell off of a step ladder. The carpenter fell onto his side as he landed onto the slab.
#4 belt slipped in the head drive, causing minor smoke, before the drive shut off. EE stated that the smell caused him to be sick to his stomach. EE worked the remainder of his shift and went to the doctor the following morning.
Injured employee had finished servicing the Cat 345LL track hoe. As he stepped down to dismount he felt a pain in his left leg & groin area. he tried to continue to work but as the night went on it got worst.
Crew was belting up they were pulling the belt tight 1/2"" chain broke and tail kicked and hit employee in the leg.
Employee was on the 703 haul truck, hit a bump in the road which caused the truck to bounce. Employee complained the seat belt buckle caused pain in his hip. He was taken to the hospital and removed from work until released by personal physician on Dec. 9, 2011. RTW next scheduled shift on Dec. 11, 2011.

The miner operator attempted to get the cable out of the roadway. A piece of rock rolled out of the rib striking him on the foot. He visited the ER and was found to be bruised. He was given a full release to return to work and did so. On 12/16/11 he visited specialist. Dr determined he had a hairline fracture in one of the metatarsal bones in his foot. He began missing work.
Employee stated that while loading rock dust onto the scoop, the rib rolled off, striking him on the shoulder.
Employee was unloading hay from truck when he fell off back of 1 ton dump truck 9 ft up. Left leg & right shoulder.
Employee stated that while attempting to remove a drill steel from a roof bolt hole on the 13 Left section. While doing so he had his left hand resting on the drill head when the steel fell and struck his hand, causing a bruise.
Fueler was attempting to dismount a Komatsu 300 excavator when he lost his footing and fell approx. 24"" to the ground landing on his back. He was transported to the hospital and released with bruising.
Employee was in the process of drilling anchor holes for tailpiece with a jack hammer when he pulled it up he felt pain in his lower back running down through his legs. He continued to work until 1/5/2012, when the doctor removed from work.
EE is a new shovel operator and was moving the shovel to another area. He was standing up and using the joy sticks to operate the shovel. He felt had a back spasm and was removed from the shovel. He was been with the company since March of 2011. He mentioned he had a snowmobile accident a few years ago and has had problems with his back before.
Employee was advancing longwall support when a rock fell from between the support he was standing on and the support being moved. The rock bounced off the panline handrail and struck employee on the lower right leg and foot.
Employee states he is experiencing neck and shoulder pain which he relates to heavy lifting and carrying at work. Employee was evaluated under Colorado Workers Compensation guidelines for cumulative/repetitive trauma, and employee does meet the criteria for an OI claim.
Driller was checking hole depths. He slipped and fell. Slipped on rocks around hole
Employee finished routine work of pump changes and took a break and shoulder was hurting. Conservative treatment unsuccessful and 4/23/2012 case changed from first aid to lwd. Patient underwen t MRI and diagnosed with degenerative changes that will require surgery.
Employee stated that while walking along the 15 Left belt he stepped off the bridge material into the mud and twisted his right knee, resulting in a sprain.
Miner reported that he slipped on plastic liner, but that he was OK. Miner AWOL for 3 days before calling in to report that he was seeing a doctor on his own for a strained back. Safety Dept notified 12/09/2011.
EE was walking bent over and hit his head in a low spot on the top.
Employee was under a D-10 dozer doing maint. When employee crawled out they felt a pain in right knee (employee has had prior issues with same knee). Surgery was done on 2/6/2012 and it has not been determined whether this is preexisting or a new injury.
Employee was picking up a top belt frame when he felt a pop in right shoulder.
AS EMPLOYEE WAS WALKING TO THE MAINTENANCE TRUCK HE FELT A ''POP'' IN HIS LEFT KNEE. BY THE TIME HE DROVE UP TO THE MAINTENANCE SHOP HE COULD BARELY WALK.
Employee stated that he was putting together 4 inch waterline, and as he was clamping a coupling together the clamp sprung back and hit the index finger knuckle and middle finger knuckle of his left hand, employee stated that he was wearing his gloves.
The employee was on the bed of a lowboy assisting in transporting a track dozer. He slipped and fell to the ground. After a few days, he sought medical attention where it was found he had fractured his ankle.
Employee's knee gave out on him, he slipped and fell 2 steps to a lower landing on the steps at the load out. No defects were noted on the steps and employee stated there was no defects to the step.
Employee was setting up a ladder and the ladder tapped a dusty pipe. Employee was looking up and dust got in his eye. He was evaluated by a physician and taken off of work for one day.
Lifting piece of used cutting out of service truck with crane, accidentally boomed up with crane causing edge to swing, striking employee on forehead, above left eye. Went on days off, started having concussion symptoms.
He was in the process of closing the rib protector door on the miner and had his left hand on the rib pinner. When the door shut. It caught his left middle finger between the rib protector door and rib pinner. This caused a fracture/laceration to the tip of his left middle finger.
EE was cutting a 10 inch pipe that was propped up on another pipe. The pipe he was cutting rolled over on his right foot and bruised it.
Mine Maintenance Welder was walking over to the metal yard, stepped on something in the mud and hurt the bottom of his right foot.
Employee's right foot was hit by a worn shell liner, while it was being removed.
The accident occurred at 6:25 AM 12-3-11. the miner were hand rock dusting the outby area when the scoop bucket crushed the victim between the scoop bucket and roof. the coal was 34 inches visibility was poor.
Employee was cleaning up around the head gate area of the longwall, a timber broke/fell and struck employee in the back causing a aggravation of an old injury. He was originally fully released from the doctor, but later additional, medical attention required employee to not report to work, lost time. Late reporting due to the later medical attention.
trying to jack a 14 person mantrip up to change a tire and strained his back
EE stated that while getting off a D-10 Dozer, he slipped and pulled a muscle in his back.
Checking for oil leak on scoop and scoop drifted forward so that the tire came to rest on top of his left foot casing a severe contusion.
Employee was using a shovel to clean in the reclaim tunnels. The shovel handle got caught in the conveyor, pulling both hands into roller between belt. The employee had surgery for fractures in both thumbs. Also a fracture in right wrist.
He was in the process of working on a shuttle car, watching the cable reel wind up. The cable struck his feet, causing him to fall backwards, striking his head on the ground. This resulted in a laceration to back of his head.
The EE was using a piece of diesel equipment (low trac) The EE had stopped the Eq. and while getting out of Eq. he slipped and fell hitting his left hand against the machine, which resulted on a break to the 3rd finger on his left hand. On 12/6/11 the Dr. determined that minor surgery would be required to repair the break to the EE's finger.
The injured employee was trying to open her car door with her hands full and a backpack over her shoulder. While attempting this she lost her footing falling on her right side fracturing her right arm.

He thought he would be Ok, but he continued to work since then, he went to the specialist and he recommended he have shoulder surgery. He had the surgery Tuesday the April 3rd, 2012. He is off work until able to return to his normal job.
Reported his right arm was sore. He did not recall anything happening to cause his arm to be sure. Through the night his arm sorer and was not getting better so he went to see doctor.

CM operator was pinned between Coal rib and standoff on the CM. He was airlifted to Charleston General hospital, no fractures, contusions only.

As employee was checking faces he stepped on a ledge in an intersection causing his ankle to roll. Employee was transported to CAMC General Hospital for treatment. Employee was diagnosed with a fracture to his ankle.

Employee reported on-going back pain. While bolting on 12/28/11-night shift, he again experienced pain. He was evaluated & referred for diagnostic testing. His release to return to work was withheld pending these results. An MRI was performed on 1/5/12 w/physican follow up on 1/6/12.At this time, he was diagnosed with 2 herniated disks & was taken off work
Worker noticed lights left on haul truck. Climbed up 2 steps to reach inside and turn off lights. Stepping back down foot slipped on bottom rung which had mud build-up slipped and fell to ground. (approx. 2') Tried to break fall with hand resulting in injury to wrist.

He has not missed any work but will be off for several weeks after surgery to recover.
On 11/23, the EE noticed pain in his right knee after working as a Brakeman all day. On the morning of 12/20, the EE noticed that his knee had swelled and had pain. He was sent to the clinic where the injury was diagnosed as a possible torn meniscus. The doctor took the EE off until a specialist could evaluate the situation.
Employee was bolting an overcast one break inby the #1 Section feeder. He had his right hand on the bar attached to the canopy when he slipped on some rock. He did not let go when he fell and twisted his upper body. His shoulder twisted in the process. Doctors gave him a few days of restricted duty.

Employee was observing roof bolting on the Main North Travel road @ x-cut #59. As the bolter was drilling a hole, the injured employee was squatted down with one hand on the mine floor watching the bolter drill, a rock measuring about 2.5"" x 17"" x 24"" fell out of the roof striking her left hand causing a fracture in it. The hand was put into a hard cast.
employee was not following our lock out tag out test out policy while working on a piece of equipment. he caught his gloved hand in a pinch point and broke his right thumb in two places with a laceration across the top. this pinch point moved because a chunk of DE caught a gate and drove it up causing an adjustment wheel to turn unexpectedly.
Was walking out of rotary dryer room when the operator hollered at employee to come back and restart the dryer. When he turned to go back into the rotary dryer building, he twisted his left knee. Surgery done on 3-7-12.
Employee was hanging miner cable for S-car roadway and when he lifted miner cable above his head got a sharp pain in his abdomen. He worked his reg. job until 1/20/2012 and had surgery for a rupture.
Cleaning out impactor, rock fell on his head from the conveyor. He got out on his own and walked to his truck. He was dizzy, and managers called 911. He was taken to the hospital and kept overnight. He has not broken bones, but suffers from some spinal compression and two broken teeth. He is at home and will seek therapy.
9:35 a.m. the employee began washing a F150 EMS Company vehicle and while walking toward the high pressure hose and valve to turn it on slipped on a icy surface. Employee fall on the concrete surface causing a left wrist fracture when trying to break the fall.

Impact wrench pinched employee's finger against track. It was found that employee had completely loosened the other bolt, but it never fell so he thought it was holding.
The man was switching his seating location in a shuttle to change travel directions. While turning around he tripped and his hand struck the coal rib when he attempted to catch his balance.
Installing a rib bolt and bent down to pick up something, when he went to raise up he inadvertently grabbed the rotation lever and his glove caught on the auger steel causing a compound fracture of the left middle finger and damage to two ligaments.

EE was striking a wedge with a sledge hammer. He missed the wedge and the hand holding the hammer handle struck against the wedge causing a contusion to right index finger and nail.

Employee was moving a 55 gal drum and lost his footing in the mud and pulled a muscle in his back.
The man was rolling a piece of rock off of the shuttle car cable reel cover when it slipped and pinched his finger.
EE shoveling clinker and transporting the clinker up an incline using a wheelbarrow. While working the EE strained his lower back. This incident was initially a first aid case. After a number of days the EE's back continued to give him problems. The EE saw a dr. on 1/4/12 and was placed on restrictions at that time. There were no restrictions prior to 1/4/12.
Employee was working on a water heater. His right foot got caught on a copper tubing and he tripped. He fell, landing on his right knee, followed by his face and then his hands. He received surface abrasions on his face and right knee and had a stiff neck and back. He was released to full duty. But on January 31,2012 he was taken out of work pending MRI and treatment.
EE was lifting 5 gallon buckets while doing inventory at the warehouse. As the day progressed EE's elbow begin to hurt. EE went to the ER and was placed on restrictions.
EE was exiting a manlift basket (type with the sliding midrail) and as he was stooping to exit he slipped in wet lime and began falling. He injured his left shoulder as he caught himself. This did not become reportable until 1/5/12 when work restrictions were implemented.
As employee was cleaning out the deck on the miner a small piece of rock fell and hit him on top of the right shoulder causing a bruise. He stated that he checked the top before starting work on the deck. He was on restricted duty for 5 days.
EE was resetting the a motor on the south E-crane. While he was doing this another EE was operating the E-crane and filling the hoppers with bauxite. When EE discharge bauxite load, the hopper shook and some rocks slid off striking EE in the right hand and the upper back area. EE placed on restricted duty on 12/28/2011.
Employee strained his back while carrying two 5 gallon buckets of material down two flights of stairs to empty them into the dumpster outside of the lab. Employee reported injury on 12/27/2011, was seen by the doctor and returned to work on restricted duty.

EE tried to step up to concrete slab 2.5' high in his effort his body weight impacted his right knee and caused sprain on his right knee. He was taken to the Medcor facility on site where he was treated as First Aid.
EE was working under a piping flange and as he stood up he struck his hardhat on the piping flange jamming his neck. There was a mound of scale build up creating an elevation change. The EE claims that the scale buildup is what caused him to strike his head on the piping flange. EE on restricted duty from 12/22/2011 to 01/03/2012. EE RTW full duty on 01/02/2012.
Employee was descending a ladder. As EE stepped down onto the floor with his left foot, he slipped. EE did not fall to the floor and did not feel any discomfort at the time of incident. After he go home he started feeling pain to his right leg around the hamstring area. EE report the incident when he reported to work the following day. EE placed on restrictions on 12/27/11.

Associate was working on bin 3 crusher with use of a contract crane service. When the crane began lifting the box one side of the box became wedged on its seating surface. One associate tried to apply pressure to one end of the box using downward pressure by pulling on the box by hand. The result was the box shifted approximately 3 inch injuring the his right hand.
EE was cleaning up a salt spill in the product reclaim tunnel when he felt a pull in his right calf
Employee was changing paddles on #4 scrubber cell when he experienced pain in his shoulder.
Employee was on the loader and was preparing to dismount, turned around and employees foot slipped off the first step and then he fell to the ground bruising right thigh and over extended his right hip, and hit his head on the fender. Employee was evaluated at the emergency room and x-rayed, no broken bones only bruised.
Employee was stepping off and marking the cut for the dragline. He was walking in soft sand when he felt a pop in his right ankle and felt pain. He was taken to the emergency room and diagnosed with a sprained ankle. He was placed on restrictive duty.
Employee was removing rear differential from equipment and the opposite end was hoisted up, forcing the end the employee was holding down, and the employee strained his back trying to hold his end of the differential. Seen at ER, released to restricted duty.
The man was carrying a roll of CO cable when he slipped on wet bottom and felt a pain in his low back. The man continued to work regular duty until 12-23-11 when he went on restricted duty status.
Helping a vendor un-strap the truck. Loosening up the binder, when it came up and struck the EE in the face.
Employee was using a hand grinder with a 4"" cutoff wheel to cut an opening on an electrical box when the grinding wheel caught a corner, broke apart, and kicked the grinder back, striking the employee in the left hand, just below the base of his pinky finger causing a laceration.
Employee was working the super bag machine when he experienced pain on the lower right side of his abdomen.
After returning from break, the 3 employees proceeded to climb back up the structure. The employees had on all required safety equipment. Once the employees reached about the 15 foot level, the complete rebar structure failed and fell over onto the North side. Jobsite was scheduled to shut down on 12/22/11 for Christmas break and reopened on 01/03/12.
EE placed chain fall onto conduit above/behind him. Chain fall fell and struck EE in the back of head. EE reported the incident on 12/18/11 to myself, EE stated that he would wait until after the holiday to see how his neck felt before going to the doctor. EE returned to work 12/26/11 and was taken to seek medical treatment and was released back to work on light duty.
Employee was resetting the cone at the crusher with a sledge hammer when he experienced pain in his right shoulder. NOTE: PER MSHA INSPECTOR A 7000-1 WAS NOT NEEDED AT THE TIME OF HIS FIRST DOCTOR VISIT OF 1/04/12. THE EMPLOYEE HAS SINCE BEEN PUT ON RESTRICTIONS STARTING 04/05/2012.
employee was spot bolting the roof, when his glove got caught on a burr on the drill steel being used, twisting his right hand and wrist around the drill steel causing strain type injuries to the wrist and a slight fracture to the right thumb.
While working in a transformer vault and installing a 3-inch high voltage electrical cable, he started to adjust the cable supports. The cable slid back into the 5"" conduit pulling his hand into it. Finger was sore and ice applied. An hour later, he went to squeeze a hook and felt a pop in his finger.
EE was getting out of a mantrip and placed his left foot under the frame of the mantrip on the mine floor. When he placed his right foot on the mine floor, he slipped and fell backwards, spraining his left foot that was under the mantrip frame.
EE got out of his truck to check the tires because he thought one blew. Three employees were pushing a frozen piece of material off of a conveyor. One employee was using a bar the other two were supporting the frozen piece while baring it off the conveyor. Once the piece was off the conveyor one of the employees supporting the frozen pieced noticed his arm was hurting.
Employee stepped backward and rolled right ankle on a metal plate covering a gap between the crusher frame and the decking.
EE did not realize that wheel he was removing was a split rim that came apart and fractured his left hand.
Employee reported incident on December 15, 2011, which he stated occurred on December 13, 2011. Employee stated that while performing housekeeping duties in the storeroom he sustained a pulled muscle in his left shoulder.
Employee was checking belts on flotation cell. He stepped over the diffuser shaft to get to the other side when his foot slipped. He was trying to catch his balance and twisted his knee .The employee did not report the incident until 12/14/11 at 7:45 am. The employee has not missed any work but is on modified duty until further notice.
Miner was using a bobcat skid steer to clean clay/debris off the railcar tracks in the plant. The bobcat came to a sudden stop when the buck caught a joint section on the tracks. The miner was secured in the bobcat with the seat restraints and the force from the sudden stop caused pain in his lower back
Operator was pushing a charging rod into a retort when he felt a strain in his neck. Operator uses a 15 foot pole to push six 50 pound bags of material into a 12 foot long retort. When the operator was pushing the 4th or 5th bag into the retort he felt a strain in his neck.
Employee was lifting wheel chock to put on front end loader and strained his left shoulder/neck.
The man was loading roof mats and roof bolts onto the roof bolter when he felt pain in his lower back.
A miner was barring down some top that was over a disconnect box. When the rock hit the box it bounced and struck the miner's right foot, fracturing one bone in the right foot. The miner had X-rays taken on 12/19/2011 and that is when the broken bone was discovered. Prior to the 19th the injury was not thought to be serious.
Employee was cleaning the restroom when she slammed her right hand in a door on 12/8/2011. She was seen for soft tissue injury that day. She was seen 3 months later by a physician for that same hand and it was determined that the injury aggravated issues associated with Dupuytren's contracture. She was placed on restricted duty with no use of that hand.
Employee was tramming bolt machine from #2-#1 entry from the deck. The T-bar hit the rib he placed his right hand on the canopy to raise himself to see. Just as he did the bolter shifted due to a downhill grade causing the fingers on his right hand to be pinched against the roof. This resulted in lacerations that required sutures. No days were missed however he is restricted duty.
Employee was walking down an embankment outside the East Ridge Dryer; the LID from a 55 gallon metal drum fell from the landing outside the control room and hit the employee on the nose causing a minor laceration (needed stitches) and a nose fracture.
Victim was removing rock from the continuous miner when a piece of rock fell between the bolts and struck him in the left shoulder and neck.
Employee was using a wrench to tighten a belt block nut on a conveyor system. The wrench slipped as he was pulling it towards him causing him to fall to the ground. As he fell to the ground he landed on his right wrist and fracturing it.
While installing a bearing the pulley shifted causing the shell of the pulley to fall off the surface it was resting on. Resulting in the employee's right and left pinky fingers to be pinched between the bottom of the bearing and the steel bearing mount surface. The employee was taken to the hospital where he received stitches in both his left and right pinky fingers.
EMPLOYEE WAS CHECKING GROUND CONDITIONS AND HAD HIS BACK TO A WALL WHEN A ROCKSLAB APPROXIMATELY 20 PDS PEELED OFF THE WALL AND PUSHED HIM TO THE GROUND. EMPLOYEE TRIED TO STEP AWAY BUT THE SLAB CAUGHT HIM ON HIS RIGHT SIDE CAUSING HIM TO FALL TO THE GROUND.
Employee was putting a screen cylinder on a storage rack and his left ring finger was caught between two cylinders that rolled into each other. Seen at the ER and stitches were required. Released to restricted duty.

EE scraped his right shin while entering cab of drill when his right foot slipped on step. This happened approx.2 months ago but employee didn't seek medical attention until 12/8/11. He was treated by a doctor on 12/8 for an infection. His wound was cleaned and dressed. No restrictions or lost time. Incident is being reported within 10 days of seeking medical attention.
While lifting tools in a bucket weighing approximately 40lbs from a gangbox the employee felt a pop in his right shoulder.

He continued to experience pain and after additional medical evaluation was diagnosed with arthritis and ACL tear which required surgery. This is not a new incident.

EE was reaching over an area guard trying to grab empty bags off the conveyor, following his attempt to remove the guard. The guard shifted when he leaned on it causing him to impact his right side against the top of the guard. He complained of pain in his right side to his supervisor
While working to move 20 inch gyp pile pipes (filled with acid water) employee was standing between 2 pipes. Back pipe rolled into back of legs pushing his legs into pipe in front of him and with feet wedged under front pipe.
Employee was filling oil of 1182 fan, when dust blew into his face. When employee backed away he strained right Achilles heel when stepping from platform.

The mechanic was installing a serpentine drive belt on a 3/4 ton Chevrolet pickup. As he pulled on a 1/2"" drive, 16"" long ratchet to loosen the belt tension, he felt a sharp pain in his left rib cage. He reported the injury the following Monday. He was taken to a local clinic where he was diagnosed with a left rib contusion and was placed on restricted duty.

During a Part 50 audit, it was determined this injury was reportable. Employee twisted his back while working on a shield in a crunch position. Cause of this injury was proper body positioning. Compliance with rules/regs, mining equip/systems not a factor. PPE (Lack of Back support) and miner proficiency may be a factor.
Party was walking into the Pilot Plant from the east door. He didn't see the forks from the forklift flesh with the concrete, at which time he tripped over them. He stumbled several times but was unable to catch himself. When he fell to the ground he landed on his Left side. An act that resulted in an injury to his Left arm, shoulder, hand, elbow and middle back.
