Accident type, without injuries

LW Shearer was cutting into the TG. Roof failed around 2 roof bolts, adjacent to Shield 176, the last shield on the face. Fall was estimated to be 4 ft. wide, 8 ft. long and 8 ft. high. The entry remained open. The fall is reported because it went above the anchorage of the installed roof supports. Ventilation was unaffected. The fall was immediately reported via the hotline.

An unintentional roof fall occurred in 5B LW #1 Entry 21.5 Wall intersection. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof.

An unplanned and reportable roof fall occurred in the #1 face at the 188 block on the F-Mains section. There were no injuries, no equipment damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

The South Portal Elevator was taken out of service due to a door chain being broken and it causing an E-Stop fault due to a tripped door switch. The elevator was brought to the top of the shaft, the door chain repaired, and the door switch reset. The elevator was tested and placed back into service. MSHA was notified via the hotline.

Two employees were trapped in the Agglomerator 1-2 elevator. The elevator company was notified and responded. MSHA was called at 12:15pm. The employs were removed from the elevator at 12:42pm.

Enough roof and ribs fell in the longwall tailgate that needed Intake CFM and face velocities could not be reached. A low intake and face velocity plan was approved from MSHA to continue mining.

The ESS cooling motor fan failed causing the hoist to lose power. This was due to heat buildup.

No. 3 Hoist #2 Side faulted on over speed would not clear fault. Points on Lilly were closed, had to disconnect Lilly drive shaft and turn Lilly back to bring over speed cam off cam follower so hoist would reset.

A roof fall was found by the weekly examiner in the 4 West Tailgate active bleeder from 76 to 77 wall. The fall is approximately 50 feet long by 16 feet wide and varies in height.

Roof in the longwall tailgate fell enough that egress off the tailgate face was restricted. Field office was notified and tailgate blockage was implemented.

A roof fall occurred 1RMS # 1 belt XC 38 entry 1. Entry had a clay vein running along left rib line joined by other clay veins in the intersection. There was no injury or property damage with this occurrence

It was reported to mine operations that one of the Westcon contractor job trailers was on fire. A call was placed to the MSHA hotline at 12:06 am to inform them of this incident. All Westcon contractor employees were off the mine site at time of incident, fire was extinguished by local fire departments and investigation began at daylight.

On 12/17/21 at approx 0900 hrs, a fall of ground was witnessed by a miner working in the area. Miners were already actively rehabbing the area and reported this new ground fall to supervision (not beyond ground support). Because the rehabbing of the previously identified ground fall, was already underway this new material that trickled out was not perceived as reportable to MSHA.

Escape hoist had an interlock for down stop preventing the hoist from normal operation. The hoist had tripped the magnetic down stop switch at the mining level but did not return back to the normal position when the cage went back to the surface.

Upon examination of the LW bleeder system there was an elevated amount of methane discovered. MSHA was notified per our approved Ventilation Plan. The mine was pulled and power was knocked in the effected area. We dispute the allegations in Citation No. 9493727. Information submitted is not an admission of any violation.

At 12:45 p.m., management was notified that the key for the secondary escape hoist had been lost. Production was stopped and all miners were transported to the surface by the primary slope hoist man car. At 1:12 p.m., MSHA was notified that the loss of this key interfered with the use of the secondary escape hoist. At 1:22 p.m., Management was notified that the key had been found.

An unplanned and reportable roof fall was discovered at the #186 break #2 entry intersection on the F-Mains section. There were no injuries, no property damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

The South Portal Elevator was taken out of service after the examination. The elevator ropes were found to be out of time. The ropes were put back in time and the tension adjusted on them. MSHA hotline was notified.

A roof fall occurred 1RMS XC 31-1/2 Entry 1. Entry had a significant clay vein running with the entry. There was no injury or property damage with this occurrence.

At 10:30 am, employee taking those measurements the elevator down stopped, leaving EE stuck in elevator between the 1st & 2nd floor. The local Fire Department was able to assist getting our employee out at 11:10 am. The employee was inside the elevator for a total of 40 minutes.

After loading material from the floor of the bench, the excavator operator was leaving their working pad to load the material from the pad itself when some material including rock and mud fell from the wall nearby, landing on the counterweight of the excavator damaging it and rendering it inoperable

An unintentional roof fall occurred in 3 East Mains #1 Entry 28 Wall intersection. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof.

The South Portal Elevator was taken down due maintenance by an elevator company. The elevator was taken out of service after multiple faults. The door switch on the west side of the car was replaced, the bottom door switches were adjusted, checked safety dogs, and adjusted the car wheels. MSHA hotline was notified and MSHA on site was notified.

Lost power on site at 1416 hours. AVISTA Power informed us that a tree limb had fallen over power line. Power back on at 1500 hours.

A ROCKFALL OCCURRED ON THE 2ND NORTH MAIN BELT 15'INBY SPAD 74+50. THE FALL WAS 30X14X10 IN THE PILLAR LINE. THERE WERE NO INJURIES FROM THE ROCKFALL. THERE WAS NO SLIPS IN THE AREA OR WATER PRESENT IN THE FALL AREA.

Contractor was cutting an opening in a rubber lined chute when the rubber liner caught fire. Contractor attempted to extinguish the fire with a water hose. The plastic screen media above the chute proceeded to catch fire which then ignited a conveyor belt overhead, damaging the chute, screen, belt and nearby components.

Mine management was made aware of a fall of ground that occurred in the 5900 west round tunnel at GMP 2940 at 7:15 am and called the MSHA hotline at 7:28 am

The Hoist Operator was preforming skip checks when they discovered that the track limit switch was not working properly.

The South Portal Elevator went down due to a door fault. The elevator was brought to the top and reset. The switch on the bottom landing and a gate switch was replaced before it was placed back into service. MSHA hotline was notified and an email sent to the FOS.

Employees were doing a shaft inspection when a bracket was identified as damaged, had to evacuate the mine and make the repairs to the bracket.

A roof/back fall was discovered in a previously barricaded heading (352-1425), at approximately 4:09 PM. The heading remained barricaded and MSHA was notified of the roof fall.

The inside flange on the inside tire of the left rear dual broke allowing it to rub on the wheel motor bolts and become hot. Once the water truck was stopped the flange rested on the tire causing it to catch on fire.

A roof fall occurred along 2L #2 beltline XC16 Entry #4. Entry had clay veins running with the entry. Additional clay vein in the crosscut joined the first in the intersection. There was no injury or property damage with this occurrence.

An unintentional roof fall occurred in 5B LW #1 Entry 24.5-25 Wall. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof. The fall area measured approximately 26' L x 12' W x 17' H.

An unplanned roof fall was discovered in the Motorbarn seals airway, between 3 and 4 seal. The fall was approximately 20' in length, the width of the heading, and fell 4' above the anchorage point. No one was injured as a result of the fall.

When the power knocked at the mine the elevator car stopped suddenly. When the brakes locked on the car it stopped suddenly and the governor rope tripped the car safety switch. The car was brought to the top and the switch was reset.

At approximately 8:05 the electricians hit the E-Stop Button on the KA1 PLC as part of their weekly testing. The hoistman tried to reset the E-Stop but the hoist would not reset and a error code was displayed on the ABB hoist system screen. Electricians diagnosed the problem and cleared the error. The hoist was operational at 9:35am.

Customer truck driver pulled into tarping area behind another truck and failed to set parking brake before exiting the truck. The driver did not use wheel chocks as required. The customer truck driver was fatally injured when customer's truck moved forward and pinned customer truck driver against the other drivers truck.

Utility operator was found non-responsive with EE's clothing entangled in the drill steel of the roof bolter.

Caught in, under or between collapsing material or buildings

EE entered feeder when gate would not close. While cutting out a piece of metal that was blocking the slide gate, material sloughed off and engulfed the EE.

Employee placed self on the bumper of the miner below the unsupported conveyor tail boom with the machine still energized. EE apparently was trying to trouble shoot issues with the stab jack by utilizing the remote control functions and in the process inadvertently lowered the tail onto self resulting in fatal crushing injuries.

Employee was underneath Conveyor Belt and became entangled.

While loosening lug nuts on a haul truck tire, an employee pinched their finger between a rad gun reaction arm and the tire rim. The employee was wearing gloves at the time of the injury.

EE was putting cover plate back on collection screw. Cover plate slipped & EE pinched rt pinky finger. EE got stitches to finger. Update: At time 7000-1 was filed, Mine understood EE had full use of finger & there was no bone involvement. On 1/3/22 MSHA issued citation, directing Mine to amend 7000-1. On 1/6/22 Mine learned EE had subsequent med treatment on or about 1/4/22.

Employee smashed left pinky and ring fingers on metal plate. The tips of the pinky and ring finger were amputated 5 days later.

The employee was working on a blower on the preheater tower which was locked out prior to working on the equipment. The employee was using an air lance to dislodge a stuck blower. Employee attempted to push the blades with hand in order to loosen them. The blades began moving unexpectedly and at a high rate of speed pinching three fingers between the blades and the housing.

Employee stated was in the process of adding 4"" aqua waterline in prep for a belt move. Employee picked up a crib block and used it to strike the end of the waterline and after several hits employee caught a finger between the crib and the waterline. On December 7th employee had the end of the finger removed due to nothing left to protect the bone.

An employee was removing a shackle from a centrifugal diesel pump lifting lug plate when EE caught the tip of the right 5th (pinky) finger between the shackle and the lifting lug plate. The shackle moved unexpectedly pinching the tip of EE's finger. The injury resulted in an amputation to the tip of the right 5th finger to the first knuckle and required surgery to repair.

Employee was cleaning inside of a Reed Gunite Pot and got left hand glove caught between feed wheel and rock shear. Severed tip of middle finger and index finger.

IE was restacking belt rails when one belt rail slid off the pile and cut the end of a finger off at the first joint. The hand was on another belt rail when it got hit by the other belt rail.

Employee slipped and fell climbing from ladder to the cone crusher, injuring lower back.

Employee was cleaning up the out slope below the ALMA seam on the left hand side of Mill Seat. When the excavator took off down the slope overturning on its side down the hill. When the excavator came to rest EE took the seatbelt off and fell out of the seat striking EE's head and shoulder.

The Employee was setting shields at shield #47 and was attempting to pull in shield 48. Shield 48 did not function as the employee attempted. EE moved to shield #46 and when EE did it began to pull in on EE. EE then tried to turn and move from harms way. EE became stuck between the pan line and the relay bar of the shield causing pain to EE's left calf area and hip.

Employee was traveling to a work area in a Polaris ranger with their foot outside of the cab. Employee's foot hit the rib causing a broken ankle.

Employee was assisting in unchoking a plugged airline for the ckd system. The employee was attempting to re-install the nipple on the airlock and inadvertent contact with the airlock causing the employee to cut the tip of EE's right pointer finger and tip of right middle finger.

EE WAS REMOVING A HOUSING CAP WITH A BREAKER BAR ON THE FLETCHER DRILL WHEN THE CAP LET LOOSE AND EE'S ELBOW STRUCK THE BOLT ON THE ENGINE DOOR. EE FELT FINE AND DID NOT NEED MEDICAL ATTENTION. 4/15/22 EE REPORTED PAIN IN ELBOW AND THAT EE HAS BEEN HAVING TROUBLE SINCE INCIDENT WITH FEELING. SAW THE DR AND WAS ISSUED THERAPY 5/9/22. FILED WITH MSHA 5/11/22.

Over-exertion in pulling or pushing objects

Employee was lowering the cover down over the engine compartment of a Lo-Trac when EE felt a sharp pain in EE's left shoulder. EMPLOYEE HAS BEEN WORKING SINCE THE DATE OF ACCIDENT UNTIL 1/31/22, WHEN EE WAS TAKEN OFF WORK DUE TO CONTINUED PAIN IN EE'S LEFT SHOULDER.

Contact with hot objects or substances

Verifying potential blockage at bottom of drag box, employee completed steps to clear blockage by normal methods. Removed flexible hose connected on top of transfer pump. While pulling hose off the connection point between a camlock fitting top of flange, connection failed. Employee received thermal burn to forearms from non-hazardous product.

Over-exertion in pulling or pushing objects

On 12/30/2021, EE reported feeling a burning sensation in lower abdomen following an attempt to break a hydraulic hose on a Loader. EE was subsequently taken to the Dr. and allowed to work pending evaluation by surgeon for potential hernia. On 3/17/2022, EE underwent surgery to repair hernia in lower abdomen.

While performing pre-op inspection of haul truck, employee slipped on ice and fell to ground.

Over-exertion in wielding or throwing objects

I.E. was hitting a pacer pump in a water tank with a hammer to clean the coal out of the pump. While swinging a hammer they felt a burn in both their shoulders. I.E. has continued to work until a doctor took them off for surgery on 8-23-2022.

Employee attempting release pallet from pickup machine when mechanism retracted and pinched finger cracking the tip.

EE had returned to work from an injury of a sprained right knee. While working near pump EE's right foot became stuck in the mud and when EE turned, it twisted and sprained the same right knee. Upon return to work EE resigned 01/04/2022.

Employee was exiting the operators cab of a ram car when their knee ""popped"" and they felt pain also.

EE was working at the Goals mine location cutting brush when a stick struck EE in the eye. EE claims EE was wearing a hard hat with face shield. EE's supervisor transported EE to Beckley General hospital in company vehicle. Beckley General transported EE to Ruby Memorial hospital via ambulance.

Struck by falling object

Employee stated EE was bringing EE's stack steel out of the top the steel came apart from each other and struck EE in the left side hitting EE in the ribs, Employee left the mine site in an ambulance for treatment EE notified us there was no broken bones doctor told EE to take 2 days off.

The employee was using a 3 lb. lump hammer to hammer a steel wedge between steel plating and the 450 excavator bucket frame, when EE noticed a small laceration on EE's right index finger. The laceration had a small steel splinter in it that needed to be surgically removed.

IE was assisting in leveling track rail. As IE was putting a tie under the rail the bar that IE's partner was using slipped and caused the rail to drop. IE's left index finger was caught between the rail and the track tie.

EE was walking from pit car and stepped into a hole. EE stated EE did not hurt just wanted to report the incident. The following day EE did not report to work. EE was sent to Company Care for an evaluation. EE is off work due to restrictions which cannot be accommodated. Similar incident occurred 10/18/21.

Employee was stepping off a carbon truck when the employee missed or slipped on a rung of the ladder falling off the truck and landing on side. The employee suffered a nondisplaced fracture of the left hip. Employee was given restrictions and put off work.

Over-exertion in wielding or throwing objects

A fully trained miner was using a track hammer to set ties when miner felt pain in miner's back. The injury was not the result of any compliance issues nor were any protective guards involved.

Struck by falling object

Employee stated went to retrieve wedges, and started to walk away when a rock dislodged from the roof striking EE in the back, sliding to EE's legs pinning them to mine floor, employee yelled for help. Co-worker arrived first and removed the rock. Employee was brought to surface, transported to Hospital and diagnosed with a fractured vertebrae and continues to be off work.

On 12/27/21 while drilling a hole to install a 6' roof bolt. The IC had a premade roof bolt leaning against the drill boom tray. Unknowing the location of the base of the bolt, as the IC lowered the drill boom it struck the bolt causing it to strike the right knee. The IC continued to work until placed on restricted duty by and Orthopedic doctor on 1/4/22.

Driver was dumping the load of ash and ash stuck in the bed and caused the tractor/trailer to roll over onto the passenger side. Driver was having neck pain and was transported to the hospital as a precaution.

Employee was working on a flatbed trailer in the garage. Employee was removing the air spring, it fell, employee reached to catch it, left ring finger was pinched between the air spring and a piece of metal. Cut/broke finger.

While getting into a pickup, employee slipped and jarred ankle. No pain initially felt. About six hours later started to feel pain.

Employee twisted EE's right knee after obtaining a stockpile sample. Employee has now had surgery which makes this reportable.

Employee stepped into some mud when EE's foot got hung. Employee attempted to pull EE's foot loose from the mud when EE twisted EE's knee. Employee continued to work up to and including owl shift 2-14-2022. On 2-14-2022 at a follow up appointment EE was placed off work until surgery for a meniscus could occur.

Employee was standing on the #7 shield located in the H1Headgate Longwall face. The shearer was cutting back into the snake, when #8 shield advanced it snagged the spare ram jack that was being stored on #7 shield, triggering the ram jack to strike EE's left lower ankle foot. EE was diagnosed with a hairline fracture of the third metatarsal.

A severe floor heave occurred 100' outby the LW face in the intake. In addition to the floor heave, the pillar rib burst. There were three miners at the end of the can line. One miner received fractures to their left leg. Another miner received stitches to the left side of their face. The third miner was check out and was fine.

Struck by rolling or sliding object

EE was cleaning by plant when material slide began sliding and hit EE in the shoulder causing injury, was taken to ER. Plant was shut down, then reinforced the material slide with anchor bolts and chains.

Employee was standing in #16 track spur waiting to chain off ballast cars. Once the cars stopped, the employee began to connect the clevis to a car when the cars moved pinching the index and middle finger on right hand.

Rocks were being removed from the continuous miner. Employee stepped up to remove the pulling rope off the rock that been pulled off machine. As EE stepped up, EE stepped on a rock this rock was sitting on causing the rock to shift and wedge EE's foot resulting in a fracture.

Contractor was sitting in service truck with door open when EE blacked out and fell to the ground striking head on the ground. EE drank coffee, chocked on coffee then passed out and fell out of the truck.

IE was using a water hose to fill the water trailer. IE lost control of the water hose when opening the valve. IE lost balance stepping off the water trailer and twisted EE's ankle. This resulted in fracture to EE's right ankle.

Employee was getting tools from toolbox. When turning to go back towards workstation, felt a ""pop"" in right knee. Entire body was not facing direction of travel causing knee to twist.

A fully trained miner was using an air bag to re-rail a dolly when the air bag struck them in the lip. The injury was not the result of any compliance issues nor were any protective guards involved.

Struck against stationary object

Employee was kneeled down plugging in an air hose. Employee then raised up and turned to leave, running headlong into the frame of a conveyor. Hard hat took the brunt of the impact, but it is suspected that their reaction of jerking their head backwards to try to avoid or lessen the impact caused whiplash to their neck.

Contractor was in the process of hanging curtain with a pogo stick when an undetected piece of draw rock fell from the mine roof striking them resulting in a fracture to their back.

Employee was testing a limit function on a drill and got EE's leg pinned between the rod support and drill steel. Employees knee became swollen. EE was seen by urgent care and discharged with instructions to rest it and ice it for 3 days.

Getting out of an Excavator the miners foot slipped due to a missing pad on the track. Employee used right hand to grab the cab and keep from falling, in doing so they hurt their right shoulder.

Employee stated, while making a belt splice on the stacker belt, the take up cradle rolled back mashing EE's left foot between the cradle and the frame. EE was transported via ambulance to ER where X-rays revealed broken bones in EE's left foot. EE was referred to a specialist. Specialist has removed from work and EE remains off work.

Fall to the walkway or working surface

Truck Driver was helping move some trailing cable. As driver was walking to the cable in the Continental Pit, they slipped on the snow and landed with left arm out stretched and shoulder was sore. Doctor gave them restrictions until they can have a MRI.

EE was looking up at plant while walking and stepped in a water hole spraining the right knee.

Struck against stationary object

Due to the weather, there was snow on the job site. The employee was doing some maintenance, cleaning out the snow/ice. The employee stood up too quickly and hit their head on a support beam.

Working on a screening plant and slipped on loose sand and millings located on the screening plant and fell off the plant.

IE was installing a curtain when EE stepped on a rock and rolled EE's ankle.

Struck by flying object

Employee was struck on the side of the face when the leveling chain broke on the roof bolter employee was operating. Employee was diagnosed with a concussion and was taken off of work.

Fall to the walkway or working surface

The miner lost consciousness for an unknown health related reason and fell injuring EE's back.

Fall from scaffolds, walkways, platforms

No adverse conditions. The fall site is a ladder and catwalk next to a log washer. This washplant is located inside a building. Environmental conditions are not a factor.

East block shifted and 4x4 dunnage collapsed; the block fell & landed on employee's right foot. Operator used the loader to lift the block off of employee's foot, and EE ""hobbled"" away from the block to a safe spot. Crushed right foot.

While doing maintenance with a pry bar and chain fall, the chain fall hook broke resulting in the pry bar slipping, striking the employee's left arm causing pain and discomfort.

Their left foot made contact with the left rear tire and pulled the rider off the ATV.

On 21DEC21 EE requested medical evaluation because EE's arm still hurt. EE was medically evaluated and diagnosed with a strain of the left deltoid muscle. EE was prescribed P/T and released to work with restrictions on 21DEC21.

Injured was building cribs on unit 2 in entry 6 when Injured picked up crib block & strained Right Shoulder. Injured was sent for an MRI on the shoulder on 12/8/21, then on 12/15/21 MRI Results revealed a tear of Rotator cuff.

While installing a cable bolt the bolt bent during the up thrust employee attempted to push the bolt up and spin the rotation resulting in the disengaging of the bolt from the chuck the bolt whipped around striking employee on the wrist fracture wrist.

Employee was adjusting the dribble bolt. The adjuster nut was not operating properly and decompressed forcefully onto the employee's left pinky. Employee was taken to the ER and given six stitches. It was also noted that the employee's pinky had a small chip in the bone tip and EE was given antibiotics as a preventative measure for infection.

Employee made a right turn off the haul road to enter the site. Upon entering the site, the employee turned sharply to the right. The truck was unable to complete the turn and overturned onto the driver's side. Employee received a cut to the top of right hand requiring stitches and extensor tendon repair of right index finger.

A mechanic was working on a truck at the 5100 field workshop and found the shifter knob had been taped with a significant amount of duct tape. The mechanic had to use a razor knife to cut the tape off and accidentally cut their left palm requiring stitches. The mechanic was wearing gloves with a low level of cut protection.

EE was removing the timing gear off of a screen. EE slid the timing gear to the end of the shaft. EE's right hand was at the 6 o'clock position. It weighed more than expected and pinched EE's right middle finger between the guard and the timing gear. EE suffered a fracture in the right middle finger, and EE received approximately 10 stitches.

A mechanic was removing a park brake from a 993K loader. The park brake weighs approx. 300lbs. and shifted as EE was positioning for removal. The EE pinched their RT middle finger tip, resulting in sutures to close the wound.

While driving a loaded articulated haul truck on a well traveled haul road during heavy rain, the back end of truck slid off maintained road onto rough terrain. EE drove truck back onto maintained road. EE complained of back and neck pain in the day following incident. Diagnosed with back sprain and placed on restricted duty.

Employee was removing hydraulic fitting under pressure. Hydraulic fluid under pressure struck employee in right side of head causing laceration approx. 4"" long requiring emergency room treatment.

Employee was climbing on the machine to start shift when they slipped. The cause may have been some mud and frost on the machine. Employee did state they cleaned off their boots as best as possible.

Employee was pulling a wrench, tightening a bolt on the pump at the plant and the wrench slipped causing EE to stumble. EE then fell onto right hand, cause a clean break to EE's right hand.

Employee was operating a loader when they ran over a chunk of asphalt in loader causing their body to shift abruptly in the cab and causing a right shoulder strain.

Miner was asked to tighten the clamp rail bolts on the outside of shaker screen. As EE turned, EE immediately ran into the handle of a ball valve at the end of a spray bar striking EE in the left eye. EE suffered a cut/laceration to the bottom of EE's eyelid. EE was released from the ER with eye drops & a follow up with a ophthalmologist.

On 12/16/21 An Operations employee initially reported to supervisor that employee's wrist began hurting while employee was using a metal bar to dislodge materials on the preheater tower. At the time employee did not seek any treatment and only wanted to report it. Employee later requested to be seen by a physician who placed the employee on restricted work which began on 1/17/22

A miner experienced left forearm discomfort after clearing excess rock on a conveyor with a 2' x 6' x 3' wooden board. The following day the miner reported pain in their forearm. On 1/4/2022 the miner contacted the nurse and was directed to schedule a clinic visit since their forearm had not improved. On 1/5/2022 the miner received work restrictions.

The production crew on shift was clearing a pre-heater tower plug in the feed pipe and stage 4. EE inserted Cardox tube into port and while trying to set the depth of the Cardox tube they felt pain in lower back on the left side. EE initially decided on self care however requested evaluation on 12/20/21 and was released to work with restrictions.

Employee was hanging the miner cable and felt a pain in EE's right shoulder, resulting in EE working shorter hours for 3 days.

An employee was shoveling material around the area when they felt a sharp pain in left shoulder.

When pushing a reject cart on the pad the cart struck a hole in the ground and stopped suddenly, muscle soreness was noticeable after returning to heavy physical activity.

Putting a track back on an excavator when the come-a-long broke causing the chain to whip back hitting EE's left hand. EE has a hairline fracture of the left thumb. EE's left hand was splinted and returned to work.

A miner was leaning out over a fall protection railing and in a downward direction and was standing with one foot upon a round scaling bar. The scaling bar rolled causing the miner to lose balance and fall approximately 7 feet to the ground below. As a result of this fall injury was caused to miner's shoulder.

Individual was working on laying out 30 foot sections of drill pipe onto the pipe handling table. Individual caught EE's hand between the drill pipe and fixed drill rod slide, that resulted in a fracture to the index and middle finger (no lacerations).

The employee was going to dump mop water outside of the Security building when employee slipped on a patch of ice from a water drip from the awning and landed on left side injuring left elbow and wrist.

When cut through, the piece of metal sprung and hit the employee's leg just below the knee on the right leg. There was some swelling at the point of impact. Employee worked the rest of the day and requested to have the leg checked out the following day.

EE was operating a shuttle car, when the car was loaded, EE exited the deck to turn around, at that moment the corner of the pillar block rolled out striking EE on the lower right leg.

Injured was stepping over a concrete slab and stepped awkwardly coming down and strained left leg. On 12/9/21 injured was seen by doctor and placed on restrictions for knee strain and sent for MRI then on 12/14/21 MRI results revealed a tiny peripheral tear of the meniscus.

While tightening a conveyor belt using a adjusting wheel, the employee felt a pull in lower back. Employee stopped using the wheel until another maintenance person arrived to help.

Employee was stepping down from a loader step when the last step broke causing the employee to fall to the ground causing pain to EE's left elbow area.

EE was going up in the man-lift basket and approaching the railing of the screen 3 and 4 deck. EE had EE's left hand gripping the handrail of the man basket, EE's middle two fingers were curled over the rail of the basket. EE leaned forward and pinched EE's two fingers between handrail of basket and handrail of wash plant.

Employee was tying the boat off to the dock. While tying the boat off EE was holding onto the dock rail with left hand, the boat started to drift away from the dock. EE lost footing and straddled the boat while holding onto the dock rail, left leg went into the water right leg stayed in the boat and they jerked against left shoulder fully extending it.

Employee was pushing a 55 gallon drum of graphite off the fork of a forklift to discard the drum into a hopper. While pushing the drum, it tipped backwards which pinched EE's finger between the drum and fork causing a laceration on EE's left middle finger. Employee received 9 stitches and is expecting to be on light duty for 12-14 days.

EE was working on curbing on the end of the conveyor line. EE grabbed a piece of grout to throw it into the bin, but the piece was in contact with EE's forearm and cut EE when EE released it.

Employee was attempting to loosen a 1"" water hose coupler by using 2 pair of channel locks. When the coupler broke loose, the employee felt a sharp pain in the right wrist.

Installing cap on the R39 pinion side with the rod bolts for the carrier and cap. Nut on bolt that holds the bearing cap. Bar slipped from holding the rod bolt and pinky finger was between the nut and cap. EE has an injury to the pinky finger on nail bed.

The employee was working at the coal mill to change the position of a cam roller; when EE stopped to take a break, EE stepped onto a 17-ton shackle. When EE stepped on the shackle EE rolled left ankle, causing a sprain.

EE was pulling a loaded dolly in the west tunnel & it fell over striking them in the area of left upper leg/hip. EE presented to the clinic with a limp favoring the left leg. No bruising or swelling noted. EE was sent offsite for evaluation, EE was modified duty for the remainder of shift. EE's regular schedule had them scheduled off duty the next 7 days, released to full duty.

Miner was trying to get silo to feed and cement eventually began to flow at a fast rate and got in the right eye of the employee.

While hanging pipe to 4"" drisco pipe to plumb in the stationary air compressor. IE tried to open the valve to check for pressure but the valve was stuck and IE had to use force to open the valve. When the valve opened, the pipe jumped forward from pressure shock and struck the IE in the mouth causing a laceration to the lip and two broken teeth.

Ee was using a chisel to remove a broken bolt from the base of a pump when a piece of bolt broke off and went into EE's left eye. Seen onsite by emergency response and requested treatment on 12/30. Diagnosed with a corneal abrasion and referred to optometrist. Released to full duty.

Ee was removing the 24"" shoe from the Tracstar HDPE fusion machine. The shoe was frozen, when the ee tugged on it, it came loose and dropped back down, pinching the ee's left middle finger between the shoe and the shoe frame. Ee was wearing hand protection. Seen at clinic, stitch required to close wound. Released to full duty.

When installing a cable bolt into the roof, the head of the bolt and the plate was still on the ground. EE stumbled and went down on left knee and struck the plate just above knee pad resulting in a cut that received 3 stitches.

Team Member was hauling water when the truck EE was operating became stuck in the snow. Loader operator cleared the snow in front of the truck and pulled it out. When the driver was exiting the truck, EE slipped and fell which broke EE's right leg.

Employee was preforming physical cement testing inside the lab. During this time the employee was removing a stud from an expansion bar. The stud became stuck so the employee decided to use a screwdriver to help remove it. It was at this time that the screwdriver slipped and punctured employee's right hand. The wound was thoroughly cleaned and a steri strip was applied.

After an inspection of a kiln, Employee was using a blow off hose to clean self off. While cleaning off the employee accidentally dropped the nozzle to the hose and when EE bent down to pick it up EE stated that EE heard something pop.

A miner's fall protection lanyard hook got caught on a railing behind EE when EE climbed down a ladder. At the bottom of the ladder EE felt the tension and turned just in time for the hook to retract toward EE, striking EE in the face and breaking two teeth.

Drill Helper was emptying a 10ft NQ inner tube. EE struck the inner tube with a dead blow in an attempt to free core and dislodged a small piece of core from the lifter case. The rock fell along the tube and struck the drill helper on the upper lip leaving a small laceration.

The injured employee was trouble shooting an oil pump. When EE turned the air supply on the pump, plunger piston moved and contacted EE's right thumb resulting in a laceration requiring 7 sutures.

EE was knelt down on stinger conveyor greasing head pulley & checking drive belts on main conveyor. Another miner started the conveyor, causing EE to fall 12' off end of conveyor. Ambulance was called with previous back surgeries as a precaution. EE was taken to Hospital and released.

EE was haul truck mentor. Operator backed in on left side to get loaded. First dipper was loaded onto HT with no issue. The shovel swung onto HT on the second pass the dutchman made contact w/ bed tail. Mentor felt some stiffness. On 1/3 the mentor stated that their back was hurting still from the contact. Mentor was issued PT & attended first session on the 13th of January.

Replacing the bottom edge plate on a push beam. After cutting the plate loose it slipped and pinned EE's left hand little finger between the plate and the frame of the push beam causing the laceration to the finger.

Injured was thawing out a water valve. Steam built up in the valve and when it released the steam went under the safety glasses and irritated the injured's eyes, resulting in a burn to left cornea. Injured was treated by medical provider and returned to work.

Miner was cutting urethane crown bar protectors to size with a utility knife. On the final cut the urethane bar shifted and caused the left index finger to move into the blade's path. The knife trespassed the leather glove and into the side of the left index finger. The miner received 12 stitches.

Two sub-contractors of Purvis Industries were installing a flange on an overhead pipe (8' high) with ""come along"" hoist. The workers were elevated several feet (not over 6') to physically fit/bolt the flange into the pipe. One subcontractor was on the extension ladder and lost their footing and fell to the ground. EE's right shoulder injury was the result of the fall.

Foreign object entered employee's left eye from above EE's safety glasses when EE raised EE's face shield. Reported to supervisor, no issues at that time. On 12/20/2021 a metal shaving was removed by a physician and medication prescribed. Employee returned to work with no restrictions.

Descending from CAT 16-H Blade while on the 2nd step with right foot, they attempted to step onto the mullboard, missed with left foot. EE attempted to lower themselves the rest of the way to the ground. Employee has both hips replaced, the stress on right leg caused a spiral fracture of the femur starting just below the shaft of the replacement hip on the right side.

Employee slipped and bumped head causing a small laceration on forehead requiring some sutures.

The operator was removing a plastic coated insulated cable hook (CAB), used to suspend a 480V trailing cable to the wire on the rib. The miner received a laceration to the base of EE's right index finger when the hanger sprung back and hit EE with the metal attachment hook.

The IE was loading 6"" x 16"" roof bolt plates onto the tray of the drill boom. When the IE turned around to go get more roof bolts, IE caught the edge of a plate on the tray with right forearm. The IE was transported to ER at a hospital where IE received 11 sutures.

Employee was maneuvering a patch piece of material and dust and debris on the existing structure fell into EE's eye, behind the safety glasses.

Injured was pulling a roof bolt from the working tray when they came in contact with the edge of a roof plate. This resulted in a laceration the the left forearm, which the injured received stitches for.

Plant utility was cleaning the dryer chute at the end of the shift with a scraper, when EE's hand came in contact with the dryer basket. EE's finger got between the hand held scraper and the dryer basket, which caused the laceration resulting in two stitches.

EMPLOYEE WAS GETTING READY TO PUT THE RIB PIN IN THE XCUT WHEN EMPLOYEE SWUNG OVER TO THE EDGE OF THE PILLAR WITH THE CANOPY AND A PIECE OF ROCK 3'X2'X1"" FELL FROM THE RIB STRIKING THE EDGE OF THE CANOPY. A PIECE OF THE ROCK HIT EMPLOYEE'S LEFT CHECK CUTTING THE CHEEK.

Employee was roof bolting when the drill steel became stuck in the drill chuck. EE took the drill wrench and hit the stuck drill steel. The wrench bounced back hitting the employee in the right eye brow causing a laceration requiring stitches.

Employee stated, while drilling hole, the finisher steel broke. EE lowered the pot to change steel. As EE reached for the pusher, the finisher dropped out of hole, striking EE's L, 5th finger between steel and pot. EE finished shift and sought treatment. EE was treated and released to return to work. On 12/28/21, EE returned to doctor and received 3 stitches on this date.

While replacing ladder cable, injured employee was instructing another employee on how to pull cable off of the drum of the winch. They were at the end of the cable where it attaches to the drum and the injured employee was standing by to secure the component that attaches the cable to the winch. The cable became free and struck the employee on the chin, causing a small laceration.

A loader operator thought EE had a leak on a 980C loader. EE parked the loader and climbed into the front right fender well to inspect for the leak. As EE was exiting the area, EE dislodged EE's hard hat on the fender, and it fell to the ground. EE bent over and picked up EE's hard hat, raised back up; and struck EE's unprotected head on the fender.

We had a breaker trip on our conveyor belt and had to check the motor wiring. EE was removing the putty tape and electrical tape form the wire lugs. EE was using a knife to cut the lugs free and slipped off cutting into EE's left hand EE was holding the cord with. EE was using a safety knife and had leather gloves on.

After employee unloaded electrical cable trays at the repulper building, and putting the forks back in the travel position to return backhoe back to the ready line, EE inadvertently pinched right middle finger where the fork pivots. After a medical evaluation EE received 3 stitches and was able to return to work.

EE put on respirator. As air was released a small piece of dirt blew around the inside of mask into EE eye. EE flushed eye and thought it was ok, but it kept irritating, so went to the clinic to double check. Clinic found only light scratch causing irritation. EE released back to full duty. Doctor put gentamicin eye drops in the eye instead of just flushing it, making it a medical

EE was cleaning the screen at the end of the day. EE had just entered the feeder box through the access door to clean the material stuck on the sides of the feeder box. As EE entered, EE knocked EE's hard hat off EE's head and at the same time a piece of rock stuck to the side of the feeder box fell and struck EE on top of the head. This caused a laceration on top of EE's head.

Employee was cutting a conveyor belt on the ground. Employee knelt down onto a nail that was still in the mat that was used to secure the conveyor belt while in its rolled up position. Employee sustained a puncture wound to EE's left knee.

Employee was moving sheet metal from one conex to another. As they placed a piece of metal on the stack and pushed for it to be in place, hand slipped. The sheet metal caught the corner of cut resistant glove, tore it, and also made contact with left pinky causing a cut.

While covering a pallet with a plastic wrap bag, the employee waved the bag in the air similar to the way one opens a trash bag. The bag caught the edge of the employee's headlamp that was mounted to EE's hardhat via elastic band. The waving process pulled the flashlight away and then snapped back in EE's face, resulting in EE's tooth cutting all the way through EE's lip.

As they were putting on the clothing they received a piece a rust into their right eye. Reporting due to I.E. being prescribed eyedrops. Employee has not missed work due to the injury.

The washdown hose on the miner was coiled up on the rear of the machine with the valve end drooping over from the top of the machine to the floor. The employee was in the process of cutting coal when the frame of the machine caught the ball valve and broke it in two. The sudden release of water pressure caused the broken shut-off valve to strike the employee in chin.

Miner was working at base of raise & looked up to see light shining down, whereupon a rock fell, hitting miner in the face, & cutting miner's lip & breaking a portion of miner's bridge. Another miner heard IE yell & ran over to investigate. The other miner saw IE covering mouth with hand, asked if IE was ok, & gave IE a rag to stanch the blood which stopped in ~10min.

Employee was moving a mechanical bolter into the heading when the brakes setup on the bolter causing the employee to contact the canopy resulting in a laceration to the lower lip and a chipped tooth. Employee received sutures for the laceration and repairs to chipped tooth. A full work release was given, making this a medical reportable.

A miner was in the process of locking down a flop gate with a bungie cord when their left hand was struck by the ""S"" hook. The bungie cord did not fully seat at the bottom of the chute causing it to snap back causing a laceration to their left hand. The miner was seen at a clinic where they received 3 stitches.

After detaching an impact idler assembly, the miner attempted to push the assembly through the center of the conveyor frame for removal. While doing so the miner pinched and lacerated right thumb between the assembly and the conveyor frame requiring stitches.

EE was was leaning over adjusting a portable air compressor hose when the quick connect ruptured striking the EE in the left ear. EE received sutures to the left ear and was released back to full duty.

Employee was going through the revolving doors on Wolf Hollow bottom when another miner forcefully pushed the door. This caused the employee's hand to be caught between the door frame fracturing right index finger.
