Accident type, without injuries

An unintentional roof fall occurred in the #4 heading on Cowan 3 Bridge at break #9. The roof fall was approx. 70' x 19' x 14. Roof fall occurred on the owl shift.

The 4703 hoist tripped out about 30 feet above the 1715 level, after leaving the 1715 during the 5 pm man trip with 56 miners on board. The hoist was reset and tried to go up again, the hoist traveled about five feet and tripped again. Electricians got it to reset around 6:30 pm and then lowered the cage back to the 1715 station. The miners were offloaded and evacuated out the #3

There was an unintentional roof fall in front of the #4 seal on 2 East. The fall was discover by the weekly examiner.

An unplanned roof fall was discovered on St. Leo Haulage at 32 block crosscut towards the belt. The fall was approximately 18'L x 8'W and 15' high. No one was injured during the fall.

An unplanned roof fall was discovered on 8 ML Belt at 31 block from the entry through the track crosscut. The fall was approximately 16' wide x 28' Long x 7' above the roof line. No one was injured during the fall.

An unintentional roof fall occurred on the MMU 001 section in #3-4 #4 #4-5 and #5 entry inby 40ft from S/S 10262 in #4 entry. The roof fall measured approx., 18'ft wide by 250'ft long and at 6ft high. There was a small cutter that came across the section which is what caused the roof to fall

Non-injury roof fall occurred at spad 3193 in the 1C Belt Entry at break 7. Fall measured approximately 40 feet wide X 20 feet wide X 8 feet high. All entrances to the fall area were timbered and a clean up plan was submitted to MSHA for review.

Around 13:00 a blaster was traveling main haul road and noticed, in a mined out room section of the back had fallen. They immediately reported the situation to the supervisor who went to assess scene with the Plant Manager and Leadman. The area was immediately bermed off and MSHA IR criteria was reviewed. A decision was made it was not immediately reportable. MSHA disagreed.

A non injury roof fall occurred in the No.3 Entry at Survey Station 3980 on the Cartwright No.3 panel. The roof fall was approximately 60 feet long by 18' to 25' wide by 6' high which begins in the No.3 intersection and travels into the left adjacent crosscut. The fall will not be cleaned up.

There was an unplanned roof fall at 4 West 20 crosscut #2 belt entry. Adverse geologically conditions caused the roof to separate and break. It measured 22' wide x 76' long x 16-18 ft. high.

The hoist man was performing the checks on the 4703 hoist for an MSHA inspector when the cage traveled beyond the lower level limits. The shoes came out of the guides and the cage would not come back up past that point. The shoes would not line up with the guides and allow the cage to travel.

Hanson Front Royal quarry contracted with Austin Powder. Austin Powder fired a routine blast in the Front Royal quarry. During the blast, fly rock escaped the quarry exiting onto a four lane highway and striking a business and individual across the street.

Excavator was feeding a burn pile on a clearing operation and got to close to pile igniting the rear of the excavator machine was a total loss no persons injured.

Employee got stuck inside the shipping silo elevator for 40 minutes. Electrical group was able to reset the elevator to get it back in service. Governor switch had tripped causing the elevator to stop between floors.

Jammer operator scraped the right rib up to the spring line to remove the bagged out wire from the previous bolt up. The jammer operator called for the loader to come clean up what was scraped out. While the loader operator was cleaning the material that was scraped down the back failed while EE was dumping material from the cleanup into the muck bay.

A roof fall was discovered on the Cumberland West Belt at 31 1/2 x-cut.

A miner became entrapped on Metz Elevator due to a glitch in power from a storm. The glitch tripped the safety and overspeed switches. The elevator was reset, and the miner was off the elevator at 8:03 PM. There was no damage to the elevator.

Unplanned roof fall at crosscut #17, entry #4, on the West Submains, at spad Station #31655. Fall dimensions 19' wide, 20' long, 6' high. Fall did not cause injury, impair ventilation, or entrap miners. Fall area developed on March 1st, 2018. Reported to MSHA at 6:35PM on 08/21/2022.

An unplanned roof fall occurred in the 11L area at 8 crosscut #1-2. Adverse geological conditions caused the roof to separate and break. The roof fell at the crosscut connection of #1-2 crosscut and into the #2 entry. It fell 60' in length and 20' wide in the track intersection.

An unplanned roof fall above anchorage zone occurred at 19 break on the #8 belt/track in the #5 entry. Ventilation was not affected and there were no injuries. This is an idle mine that is being maintained.

An unplanned roof fall was discovered on ML #3 Belt in 00 Block crosscut towards the return. The fall was approximately 18'L x 23'W and 6' above the roof line. No one was injured during the fall.

Roof Fall at East 2 - 28 to 29 block - #1 entry. Measures approximately 40 feet long by 16 feet wide by 5 feet high. This was a transition area that consisted of laminated strata.

An unplanned roof fall occurred on 8/19/22 at approximately 20:05 hrs. The fall occurred at crosscut 56 of the slope belt line. No persons were injured as a result of this fall.

A haul truck was in use in a sand quarry when a fire started in the engine compartment. The operator immediately shut the truck off, exited the cab, dismounted the truck, and distanced self safely from the machine. The fire was extinguished by local EMS. The extensive damage to the front-end of the truck is expected to result in a complete loss of the machine.

Unintentional roof fall at Brk #49 in the #2 entry of South Mains Section. Breaker support and danger signs were set at all approaches.

The SP Material Hoist was taken out of service due to a brake not releasing. A bolt was found to be out of the brake plunger. It was replaced, the hoist tested, and placed back into service. MSHA was notified via the hotline and on site.

Due to arc flash incident (loss of power) in T2-3-1, Central Control was unable to complete the H46-B1 emptying procedures. This allowed pulverized coal to sit in the bottom of the coal surge bin. The coal then caught fire.

An unplanned and reportable roof fall was discovered at the #3 block in the #4 entry on the West Mains intake. There were no injuries, no property damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

70 ton feeder on the surface was full and a skip was loaded so it could not dump into the feeder. Hoist was still technically operational.

A Non-Injury roof fall accident occurred in the No. 2 entry at crosscut # 10 on the MMU. No.005-0 Section during retreat mining, which stopped mining operations due to the remote control units would not operate the mobile roof supports to tram the units back away from the pillar line.

A roof fall was discovered at 9-21 on the haulage. The fall is being cleaned and re-supported.

Hot and dry conditions, coal spontaneously will combust and the fire wasn't put out with in the 30 minutes. Fire was put out within an hour.

While traveling into 7RT Bleeder, roof fall was found. The top at crosscut 18 had weakened and fell. The measurements are 20 feet wide, 20 feet long, and 10 feet high at the middle of the fall. Area is not wet. Area was bolted with 7 foot rebar resin grouted bolts. Ventilation was not disturbed.

An unintentional roof fall occurred in 3 North Mains 12 Wall #1 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and a lateral shift of the strata causing the 8' bolts to fail at their couplers.

A fire occurred at the Goldstrike tire laydown which involved numerous used tires. A set of damaged tires that were removed from a piece of equipment were placed in the used tire laydown. The tires are believed to have been damaged and hot enough to self-ignite causing the fire.

An alleged ignition occurred on the 3SLW. The HG drum sparked and it appeared to the operator as an ignition. There was no evidence, no bad bits, all water sprays were in good condition, and plenty of air on the face. It was determined there was no ignition. This was investigated with no evidence of an ignition by MSHA.

Water Supervisor found this roof fall at 6:30 am while investigating a water issue. This intersection was mined 5+ years ago. This intersection has little to no water in the roof strata. Roof condition in adjoining entries look good. This roof fall is not in a haulageway, travelway, or escapeway. It does not impede weekly examination, and does not affect ventilation.

An unplanned roof fall has occurred on Alma #3 belt in the #3 entry, crosscut 1-2 that is approximately 50'long by 16' wide by 7.5'.

There was a roof fall above anchorage point in the belt line of 22E at wall 3 to 4. There was one geological feature present in the area. The length of the fall was approximately 80 feet by 20 feet. It was around 10 feet above normal roof.

An unintentional roof fall occurred on the MMU 001 section in the #3 entry at S/S 10603. The roof fall measured approx. 18' wide, 15' long, and an undetermined height. There was a large cutter that came across the section, which is what caused the roof fall at the face.

Electricians were performing annual battery replacement for the KA1 control cabinet. While replacing the batteries for the hoist controller PLC UPS the breaker feeding the hoist controller PLC was tripped. When power was restored B11.28 I/O module was in error and was causing the hoist trip under several erroneous faults.

An unplanned roof fall occurred in the 6 south area at 49 1/2 wall in #4 entry. Adverse geological conditions caused the roof to separate and break. The roof fell at the crosscut connection of #3-4 crosscut and outby entry. It fell 15' into the 4-5# crosscut and inby entry.

An unplanned and reportable roof fall was discovered at the #4 block in the #5 entry on the E-Mine slope. There were no injuries, no property damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

MSHA & WVMHST Officials were notified of the potential of a roof fall occurrence at approx 8:30 am when 2 ft of rock was discovered on the track/belt entry of the Winifrede Slope at the 1100 ft mark and the entry was ""working"" At approx 9:00 am the roof fall was discovered by management and an MSHA Official Areas were dangered off and a clean-up plan has been submitted for approval

The west hydraulic brake for the #3 hoist was found to be out of spec during the static brake test. The east hydraulic brake and pinion brake were still functional though. MSHA was notified that the hoist was down bc of this but due to redundant braking mechanisms, the hoist was not actually down and there was no need to notify MSHA.

At crosscut 46 in the north mains a roof fall was found between entry 2 and 3. The fall was approximately 35 feet in length by 18 feet wide. The fall was supported with 8' bolts. The entry height was 10.75'. The height of the fall varied throughout the fallen cavity. The thickest portion of the fall was 9' thick.

Electricity to the mine was lost due to a storm. As a result, the production hoist was inoperable.

A roof fall was found at 11-23 crosscut from track to belt. The fall was approximately 40 feet in length by 16 feet wide and 12 feet in height.

A miner was trapped on Miracle Run Elevator while riding out of the mine. A rope gripper fault was the cause of the elevator to stop. Another EE arrived at the portal, and reset the elevator. EE was off at 3:40 AM. The elevator was test ran and put back into service.

The SP elevator was taken out of service due to the ropes jumping out of time. The ropes were repaired, rope guides added, the elevator was tested, and the elevator was placed back into service. MSHA was notified via the hotline.

A roof fall was discovered at 12-29 in the return entry. The fall is being cleaned and re-supported.

An unplanned and reportable roof fall was discovered at the 115-116 block in the #2 entry of F-Mains. There were no injuries, no property damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

A coal wall fire started from spontaneous combustion, in an inactive area of the mine property. It burned for over 30 minutes, due to difficult access.

An unplanned roof fall was discovered in the 1 South/1 West Airway. The fall was approximately 12' wide x 16' long x 6' above the roof line. No one was injured during the fall.

The Dry Ridge Elevator was taken out of service for an extended period of time to address equipment fault alarms. The Main Drive computer as well as all encoders were changed to correct the issue

At 3:00 p.m. management was notified that there was a mechanical issue with the emergency brake on the secondary emergency escape hoist and it was being taken out of service. MSHA was notified at 3:27 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the secondary emergency hoist was put back in service at 6:09 p.m. on 8/04/2022.

Three contract employees became stuck in the freight elevator. Electricians were called and they could not reset the elevator. Notifications were made. ERT rescue team was contacted. Attempted to open doors and hatch would not open. CE's were finally able to be released from the elevator at 10:52 a.m.

The Coal Fork Main mine hoist was down from 9:30am until 11am on August 2nd due to a contactor in the hoist house going bad. The contactor was replaced and the hoist is working as of 11am on August 2nd.

At approximately 5:16 pm 8/2/2022 is was reported that small fire approximately 4 miles east of mine office was burning on the mine property. DNRC was on site fighting the fire. This is a very remote area, with no mine activity in the area. Called MSHA report line at 5:26 PM. There are no structures or equipment in the area.

A water inundation occurred due to a Flash Flood from a rainstorm. The Flash Flood washed out a dam in the drainage system, on the surface, which allowed water to enter the mine. The water created holes in four stoppings, one Overcast and one Box Check. No major air changes occurred and no one was injured or entrapped.

A ROCKFALL WAS FOUND IN THE LEFT RETURN OF THE #3 UNIT IN THE 2ND NORTH MAIN AT SPAD 92+00 TO 93+40. THE ROCKFALL DAMAGED A LEFT RETURN BRATTICE.THERE WAS NO ONE INJURED IN THE ROCKFALL. THE FALL WAS APPROXIMATELY 100X19X12.

An electrician was working to diagnose a problem with encoder faults on the production hoist. A service specialist remoted in and advised making a change in the diagnostic screen. This change took control away from the hoist operator and made it inoperable when in automatic.

A roof fall above anchorage was discovered outby in the No. 6 entry at the intersection of Spad No. 4357.

Third party contractors working for another contractor company. Welders were welding on Belly Pan of Shaker with a 6""X6"" patch on outer plate of Belly Pan. Inside of belly pan was lined with rubber lining. Rubber lining ignited from contact on same plate of Belly Pan. Engulfing Shaker completely.

Contractor suffered fatal injuries while performing maintenance on ""Dutchman"" of electric shovel bucket. Fatal injuries resulted from being pinned between shovel bucket door and bucket.

A miner was fatally injured when the supply car on which they were sitting was struck by a locomotive, causing them to fall where they were crushed between the coupler and deck of the supply car.

Employee was servicing a dozer alone with the machine running & it was somehow put in gear & started moving. Before the employee could get out of the way the machine trapped employee's foot & started pulling employee in between the track & the cab, which led to employee being run over by the machine. This is our best guess as there were no actual witnesses to the event.

Foreman injured right ring finger while assisting a vendor unload 250 gallon tote of oil on 08/29/2022. Medical treatment given to injured meets criteria of 50.20.3. Foreman has metal cover on injured finger and flinger is currently bandaged and immobilized. The injured has made follow-up appointment with orthopedic surgeon.

EE was rolling stone back from splitter mouth when the material continued to roll back smashing EE's left thumb between multiple pieces of stone

Struck by falling object

A mantle liner was being prepared to be hoisted to another position - rigging from the crane caught one of the ears on the mantle, knocking it off the cribbing onto employee's foot. Foot was not amputated at the time of incident.

Over-exertion in lifting objects

Employee was lifting a rail up to the roof and felt a pain in stomach. Employee went to the doctor and was diagnosed with an umbilical hernia. Employee has not missed any days of work or had any restricted duty days. The doctor said that the employee may not need surgery.

Caught in, under or between a moving and a stationary object

Miner was assisting a Mechanic with the removal of an hydraulic cylinder. The cylinder slipped and it crushed the tip of the Miners right index finger.

Fall down stairs

Employee fell down steps trying to get feeder shut off to stop overloading conveyor belt.

Over-exertion in wielding or throwing objects

IE was disposing of used filters from the RO plant into a dumpster. IE lost EE's grip on a filter and still tried to throw it up into the dumpster then felt a pop in EE's left side rib cage area. IE was transported to McCleansboro ER. IE was examined and given three days off, then a full return to work release. IE did has returned to work.

Struck against a moving object

Employee aggravated lower back / Left buttock from operating the truck over time. Employee was looked at by our occupational doctor on 8/30/2022 and given time off, incurring an LTI.

Over-exertion in wielding or throwing objects

On 8/31/2022 IE reported a strain to the abdomen that was caused by throwing over a miner cable. IE was taken off work for hernia surgery on 5/24/2023.

Struck by falling object

Employee was cleaning rock off of the miner. When they slid one off it hit their leg causing a laceration. They received 11 stitches.

Struck by falling object

Employee was assisting with bolting a gopher hole when a rock fell from the roof and struck EE on the left and right wrists, causing a laceration to both forearms and a fracture to EE's left wrist. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Struck by powered moving object

The employee was washing the tracks and was called on the radio and notified that the railcars were being moved to stand clear. After the 1st & 2nd car passed the employee was snagged by the 3rd car and thrown to the ground breaking EE's L knee cap.

Fall from machine

EE was crossing the train to get earplugs. When jumping down EE twisted EE's ankle.

Struck by falling object

Drive crew employee was working and accidentally bumped hardhat against a roller that was above the employee. The roller dislodged and fell out and hit employee's hand causing injury.

Struck against a moving object

The mantrip that the employee was sitting in was struck by another mantrip traveling in the opposite direction. The injured employee braced themself and felt a pain in the right shoulder from the sudden movement. The employee worked all scheduled shifts until employee was taken off of work on 11/16/2022 to surgically repair the right shoulder.

Over-exertion in pulling or pushing objects

Employee performing maintenance under the cone crusher and while pushing on a roller felt a pop in right knee. Initially declined medical evaluation but soreness persisted

Employee was installing belt structure and pinched hand between two hard surfaces.

Struck against a moving object

The mantrip that the employee was sitting in was struck by another mantrip. The injured employee braced themselves and felt a pain in right shoulder from the sudden movement. This employee worked all scheduled days until they were taken off of work on 10-4-2022.

TRYING TO MOVE A PUMP WITH GIN POLE. THE CHAIN ON THE GIN POLE BROKE AND HIT THE EMPLOYEES.

Over-exertion in lifting objects

Employee was moving a miner cable out of the haul road when employee felt a pain in right shoulder, employee continued to work until 09/07/2022 and the doctor took employee off of work.

Fall from machine

EMPLOYEE WAS STANDING ON TOP OF THE BELT SLIDE WHEN EE SLIPPED OFF THE BELT SLIDE. THE BELT WAS DAMP AND SLICK. EE HAD PAIN IN EE'S LOWER BACK AND RIGHT LEG. EE WENT BACK TO THE DR. ON 9/8/22 AND DID NOT REPORT TO WORK ON FRIDAY 9/9/22. THE ACCIDENT BECAME LOST TIME ON FRIDAY 9/9/22.

Fall to the walkway or working surface

Employee was in the surface shop working on a piece of equipment. EE was on EE's knees driving a pin out with a sledge hammer. EE was swinging to hit the pin and missed the pin becoming off balanced falling on EE's right shoulder and arm. Incident occurred on 08/29/22 EE worked until 09/27/22 when EE was taken off work for medical treatment.

Over-exertion in lifting objects

While moving a piece of low structure employee twisted back causing pain in lower back and back of legs, resulting in a lost time accident starting 09/02/2022

Fueling a piece of equipment & caught self with left hand/forearm & right hand when EE started to fall off the equipment step when reaching for the fuel tank step. Initial doctor's assessment was that it was a left shoulder sprain. Further assessment on 10/11/22 indicated surgery was necessary. EE will work at full capacity up until surgery on 10/18/22.

Struck against stationary object

Employee was replacing 1 1/2"" feed pipe lines on the spirals. Employee's head contacted I-beam while grabbing material for the task.

Over-exertion in lifting objects

Employee was lifting samples of coal at the tipple from the sampler. They felt a pop in their back. The employee finished their work and then let supervisor know of the incident.

Trying to move pump using a gin pole. The chain broke and hit the employees.

Struck by falling object

A fully trained miner was taking down a wall when a piece of rock fell and struck EE in the neck. The injury was not the result of any compliance issues nor were any protective guards involved. NOTE: Employee did not begin missing work until 9/6/2022.

Employee was installing belt structure and pinched EE's hand between two hard surfaces.

Fall to the walkway or working surface

While moving dill casing, injured person (IP) was spotting the forklift. While walking from one side of the forklift to the other, the IP tripped on the un-level ground and fell. IP put EE's arm out to brace for the fall and resulted in injury to left shoulder.

Employee was performing longwall tear down duties on the face and slipped and fell on a wet surface, injuring wrist.

Fall from machine

Employee was walking down the drivers side walkway of an excavator, missed step with right foot falling to the ground. Fall caused broken ankle, femur and injured hip to right side of body. injured employee failed to use OEM handrails(3-points of contact) when lost balance.

Haul truck operator was rough loaded when large rock was placed in bed of truck.

Employee was using hammer on conclave liner to remove it. EE placed EE's foot under the liner and was injured.

Over-exertion (Not Elsewhere Classified)

Employee was moving the section when they slipped on the conveyor belt twisting their right knee which resulted in a meniscus tear.

Over-exertion in lifting objects

EE was hanging miner cable on the section with 2 other EE's when EE felt a pain in EE's right arm. Later in the shift EE's fingers on EE's right hand started becoming numb. EE was using proper lifting techniques and had PPE on. The cable had tension prior to lifting.

Absorption of radiations, caustics, toxic and noxious substances

Employee was stung by insect twice in Brown House at Butte Fuel Dock.

Over-exertion in lifting objects

A fully trained miner was loading belt when miner felt a pop in shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

While walking to EE's dozer employee tripped over EE's feet, hitting the bottom of the manlift with EE's left arm. The date of the incident the employees arm was x-rayed and no fracture was reported. On 8/31 an additional CT scan was performed, which now has reported a fracture.

IE was working on a CM under an area light lid when the lid fell striking the employees right arm. The IE was brought to the surface and taken to the hospital for medical treatment.

Employee stepped off of the bolter & identified a hazard. EE had a rock in the travel path, so EE picked it up & was placing it down by the rib. It rolled pinching EE's right gloved middle finger resulting in a laceration. Site medic referred EE to ER. Two sutures were required, full release was issued making this a medical reportable.

Employee was cleaning inside t-1 #4 vacuum header line, when a plug of iron ore concentrate and water released from the pipe, forcing the material and the employee out of the pipe. The employee was ejected about 15' out of the pipe, while falling 6' vertically to the concrete floor below, resulting in fractures to the left arm, right femur, and pelvis.

Injured employee was trying to remove the tires off of a shuttle car and EE's right shoulder started to hurt while performing this task. Employee didn't violate any rules or regulations and has training & experience.

A piece of draw rock fell from the top about 4 inches thick 22 inches long and 20 inches wide hitting the employee on top of the head and stubbed their neck up. The employee was in the break between 3 and 4 on the section. There were no witnesses and the employee came outside and felt like their neck was still hurting so they went to the Medical Center.

While moving hoses and valve, reaching around differential, crossmember and frame, twisting and pulling, Employee left shoulder had a very sharp pain on the top and down the front of arm. Became a recordable on 9/8/22 when physical therapy started.

While offloading structure on the belt line, EE felt a burning feeling on the inside part of right shoulder. EE was taken off work on 08/31/2022 for surgery.

Employee alleges the day before they were rolling some belting around but they didn't feel any pain until the next day employee wasn't sure if they injured themselves at work. Later after seeking medical attention they stated they did hurt their lower back at work. This is late reporting do to just receiving Dr. notes.

Tail Pulley on conveyor was plugged and stopped the conveyor. Conveyor was locked out and gear box brake was set to prevent the belting from moving backwards. As material was removed relieving pressure, belting moved forward pulling miner into tail pulley.

Employee was setting up to bolt when the other operator lowered the front of the roof bolter causing the canopy over EE's head to strike EE on the top of EE's head resulting in a jammed neck. EE's physician would not give EE a full duty release.

Employee had removed the suction plate on the bottom of the roofbolter drill pot and was in the process of re-installing this plate. While installing the plate it slipped out of their grip, falling approximately 16 inches contacting their shoulder. Employee continued to work until until 08/10/2022 when they were placed on restrictive duty by a doctor due to discomfort in shoulder.

As the suction line was being removed at the Primary scrubber 10 PU 1 pump, hot steam and water was released out of the suction line injuring 3 employees and 1 contractor.

Main water pump at mine had blockage. Employees of the mine along with one contractor employee were taking the pump apart to remove the blockage. Heat/steam had built up pressure in the pump unknowingly and released when the bolts were removed from the flange on the pump. Result was 2nd degree burn on 25% of body of contractor employee.

An electrician was kneeling and when EE stood up EE felt knee pain. EE was assessed by doctors and released back to work. An MRI and follow up appointment were scheduled. EE was later diagnosed with a torn meniscus and began missing time on 08/11/2022.

Draw rock fell from mine roof striking employees leg while operating side controls of roof bolt machine. Employee continued to work until 08/18/2022. Was determined to be reportable injury on 08/31/2022.

Employee was running excavator in shot and when EE dropped the bucket into the shot it jarred EE. The sudden jar caused a tear in EE's perineum and it started to bleed. EE also complained of back pain. EE went to the ER and was released to see EE's family doctor. No stitches.

Miner was pulling an extra long length of hose off of a reel at somewhat of an angle. As miner's body was positioned sideways, miner felt a pop in right knee resulting in an injury to right MCL.

At the end of the shift they went to the doctor to be examined. Employee was diagnosed with a broken rib.

Employee unhooked a chain that was holding up a bundle of wire mesh roof screens that were leaning against the rib in the 12th north section. The bundle of screens fell over and hit the employee and knocked EE to the ground.

Once the cut was completed the employee pushed the cut piece away from them and employee said something hung their glove and pulled them out of the manlift. EE proceeded to unload rollers and belt structure at the 9 West longwall for several hours prior to EE mentioning that EE felt any pain as a result of the collision. EE was bending and throwing out bolts on the helper side of the twin boom when EE turned to go back towards the back of the machine and the operator trammed forward and EE's toes got under the cats of the machine resulting in two fractured toes and sutures.

The EE was working in 2W Longwall teardown. The injured was using a socket and wrench to remove bolts from old pan line. While doing so, EE felt pain in the left shoulder. EE worked till 8/4/22, then began missing. RTW on 8/8/22.

Tandem Coal Truck driver was attempting to change to a higher gear on a slight decline. When driver changed gears, EE thought it had gone into gear but was actually in the neutral position. When the driver realized the truck was in the neutral position, the truck had gained speed causing the truck to runaway.

On August 30 employee was operating the wet plant loader for the majority of their shift. August 31st, they called in stating their back was hurt. On September 1st, they brought in paperwork stating that they had gone to the hospital for medical care. They then informed me that their back was hurting because of riding the loader on the shift and it was not a specific accident.

Employee was conducting a PM on a boom truck and fell to the ground, causing a pulmonary contusion, fractured clavicle, and rib. Employee was taken to the ER, hospitalized overnight, released and is currently off work. ONGOING. MSHA was immediately notified out of an abundance of caution.

Employee was closing a product slide gate, which was held open by a piece of steel. The employee used left hand to cut the steel, unfortunately right hand was near where the gate closes. When they cut through the steel the gate slide down and hit the tip of finger on right hand.

Shoveling hopper out when hand slipped off shovel handle and stuck side of bin, causing pain and bruising in right fifth (pinky) finger. Examination showed no breaks but finger was splinted to protect finger.

The operator reported they received a ganglion cyst on their right hand from clearing material from the CV-2 chute hopper using a breaker bar. They waited to see whether it would go away on its own and it never did. The EE never received prescription medication or restrictions. EE found where the leak was coming from and went to pinch the hose so EE could shut the gas tank off. Employee was using a loader to dig out big rocks. One rock was halfway out of the ground and EE was digging and ramming it with the loader's bucket, when EE felt pinching/pulling in EE's back. As the day went on, EE felt pain getting progressively worse.

As employee was pushing the drag drum dirt pan into place, EE felt a pop in EE's left elbow. First aid was administered. However, injury required surgery on 09/21/2022.

Improper use of hand tool. While unplugging a DTH, EE used a 48"" pipe wrench as a hammer. While striking the DTH, the crook EE's right hand was pinched between the hook and handle causing a puncture injury requiring 3 stitches.

TEAM MEMBER WAS LIFTING INSPECTION DOOR TO VSI. WHILE MOVING TO GET A BETTER LEVERAGE ON THE DOOR TO USE TWO HANDS, THE DOOR SLIPPED AND LANDED ON TEAM MEMBER'S LEFT THUMB. THE EVALUATION BY THE HAND SPECIALIST INDICATES NERVE DAMAGE AND BONE IS OFFSET AND WILL NEED SURGERY.

An employee was loading material from the 308A series, while leaving the pit to go to the fill. The seat accumulator broke off the bracket and the seat bottomed out, injuring the employees lower back.

While operating a skidsteer the equipment overheated. The employee exited the equipment to inspect the issue and while the engine compartment was open the radiator overflow line had whipped up and a burst of hot coolant came out hitting their hands. The overflow hose was not secured in place due to the mounting brackets being on the wrong side of the radiator frame.

Shipping employee placed hands in harm's way while closing silo elevator doors. Employee attempted to use nylon strap which was broken and too short to pull doors together. In the process, employee's right hand was caught between the upper and lower doors.

As the two were both pushing on the bars, the injured EE lost control of bar and slipped off the radiator causing EE to fall to the ground. When EE fell, right knee impacted the concrete floor of the shop. EE immediately felt a pain in lower back, side, a leg.

(Sutures were required in EE' pinkie finger)

On 8/18/22 The employee was collecting the shank and tooth (approx. 60 lbs.) from the loader that had been collected at the belt magnet. As they pulled the shank off the belt, which was shoulder height, their fingers slipped out of the tooth and the shank fell causing the employee to ""catch"" the weight with their left arm. This caused muscle strain in their left shoulder.

EE reports EE was trying to pull the guard off of a counter weight due to material build up. Once the weight came loose EE reports EE fell backward and stuck out EE's left hand to brace self. EE is right hand dominant and has no open wounds from EE's fall. Resulted in closed fracture.

They did x rays and determined it was a sprained ankle with no fractures.

Employee stumbled while walking causing head to strike a roof bolt plate. Resulted in a laceration requiring 14 stitches.

EE was doing maintenance on equipment, Corner fell out and hit EE in the right foot and back 12"" tall 24"" wide 6"" thick piece of rock/coal.

On Wednesday August 24, 2022 the operator of a 2DP600 forklift was returning unloaded from the truck loading area to the packaging area. While transitioning between 2 warehouse bays the driver was distracted by nearby activity and drifted into a support column. A piece of broken mirror came in contact with the driver's forehead causing a laceration that required 7 stitches.

Employee was repairing our 988B teeth. While employee was hammering in the teeth, a shard of metal came off of the teeth and entered employee's arm.

The technician was using a ""RAD"" gun to torque bolts when the torque gun slipped, crushing the tip of the tech's middle finger on right hand.

IE was grading out of the tail-gate when a roof bolt was kicked out by the lump breaker striking the IE above the left eye causing a laceration that required stitches to close the wound. It also caused a tooth to be chipped.

While tramming on ledge 2 after finishing boulder drilling, the drill got stuck. EE tried to tram and the drill went up on a rock giving a tipping feeling. EE moved back off the rock and got out of the cab to move it out of the way of the track. EE lifted and rolled the rock. It moved unexpectedly and pinched EE's left index finger cutting it between the first two joints.

The employee was tightening the nut on the head section of the 7'standard crusher. A suspended weight and flat bar were being used to conduct final tightening. The employee was on the other side of the head and they did not realize the tip of finger was in one of the bolt holes on the bar. When the bar was struck, the bolts broke and the bar shifted cutting the tip of the finger.

EE was greasing the bucket of the unloading crane at the down river dock when EE reached over the bucket and caught a burr (metal shaving) on the forearm causing a 9/16 inch long laceration to EE's left forearm. EE received 2 stitches and a tetanus shot.

Employee was working on the continuous miner, was removing the side lid covers for the panel. Cover slipped out of one hand causing EE's right index finger to be caught between the cover and the rib.

Employee stepped off equipment ladder stepping on uneven terrain causing inversion sprain to left ankle.

EE was cutting a hose with a knife and it slipped resulting in EE cutting their front right thigh. 3 stitches were required.

Hand was on canopy when rock fell from mine roof smashing middle finger on left hand. Resulted in a chipped bone on tip of finger.

Employee was walking back lo-lo to shovel. They reached down and put a hand on the lo-lo to help walk and a shard of metal cut left pinky though their glove

Employee was installing cathodes into cell tanks, Cathode got hung up, employee tried to move Cathode by hand resulting in smashing EE's finger in a pinch point. Employee had Laceration on EE's left middle finger. Employee was taken to emergency room and was given a numbing medicine and stitches.

Employee was bolting up a round utilizing a jackleg when EE was contacted by small rock causing laceration to lip. The laceration required sutures to close and the employee was issued a full release.

The injured was helping to change out the 11 East LW head Drive power unit and as EE checked to see if head motor and hub would clear the drive housing the winch rope moved suddenly, jarring EE's left hand causing pain and swelling. On August 31, 2022, the injured was diagnosed with a fractured bone in EE's left hand and was given a wrist brace to wear.

Employee was doing a pre-shift inspection on a Cat D5H dozer. EE was checking the anti-freeze level and had opened the radiator cap. The dozer had not been started and was cold. When EE looked down at the top of the radiator, a piece of dust or dirt blew into EE's right eye. EE was later diagnosed with a cornea abrasion.

Employee stated that EE was traveling with an inspector and they were going through a mandoor. EE was holding the door for them to go through and after the last went through EE turned to go through. EE's hand slipped off of the handle and the door slipped out of EE's hand causing the door to smash EE's right thumb. Employee received 4 stitches.

Ee was jostled in the cab of EE's haul truck when shovel operator dropped a load. Seen on 8/31 for initial visit, diagnosed with a lumbar and thoracic strain, released to full duty. On 9/12/22, ee referred for physical therapy, making this reportable for treatment.

Employee operating an extended reach forklift was pushing the Point Sheave into the bore that accommodates the weldment lock. An employee was holding the weldment lock plate. The fork slid off and pinched the top of the employee's fingers between the shaft weldment lock plate lock and the fork. The employee suffered an injury to the tip of #4 digit of the left hand.

Employee was working on the air actuated knife gate. The ram that attaches to the gate was disconnected from the gate. The gate was in the down position with the ram being in the up position. EE was spinning the piece that attaches the ram to the gate up the threaded part of the ram and the water pressure coming into the freshwater tank pushed it up into EE's finger.

Struck by powered moving object

Employee was guiding a skid steer up to a conveyor to push it. The Injured worker advised the skid steer operator to pull back and turn to the right, but the skid steer turned to the left and struck the injured operator in the right calf with the edge of the bucket attachment.

Employee stated EE was changing out a piece of 60"" top structure on surface #2 belt. EE stated EE was pulling an old piece out of the belt line when it slipped out of EE's hand allowing the structure to smash EE's hand between the rail and the structure. (On 8/25/2022 employee was diagnosed with a broken 5th digit ""pinky"" finger.)

Fall from scaffolds, walkways, platforms

Employee was walking down a concrete ramp when they detoured causing them to trip on another ramp.
