Accident type, without injuries

The # 8 elevator was out of service for a period of time exceeding 30 minutes due to the malfunction of the RO coil @ 8:48 AM. The RO coil was replaced and the elevator was placed back in service @ 10:20 AM. No persons were on the elevator during the outage. Escapeways were re-routed underground.

A roof fall was discovered in the 2 west 1 mains, 59 crosscut, between #1 & 2 entries. The fall measures approximately 25 feet long, 18 feet wide and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts.

There was a hot spot in the precal coal fines bin that was stirred up. The stirring conditions were created when attempts were made to withdraw the hot material to the HGG and when the Pre-Cal Coal Mill DC started. The start of the DC introduced new aerated coal into the bin. At this point all conditions were met to form the unplanned ignition.

An examiner and an MSHA inspector found a roof fall during a weekly examination of a worked out area. The fall was located in the 1 left, 1 right, 3 left, 4 West panel in entry #2, between crosscuts 11 and 12. The fall was located in the examiners normal route of travel.

During the shaft inspection, a defective slow down switch was found. The mine was evacuated and the slow down switch was replaced with a new one. The hoist was tested and miners returned underground at 11:00 a.m.

A cave in the gob behind the shields of the 2LEM longwall (MMU001) occurred that impaired ventilation by blowing down 2 curtains. This was corrected by re-hanging the 2 curtains in the intake entries. Further evaluation revealed that the floor heaved and material fell from the ribs into the tailgate walkway and escapeway. The material in the escapeway impeded passage.

Our examiners found a roof fall during their weekly examination of the 4 West return air courses. The fall was located in a crosscut between entries 1 and 0. The fall was located adjacent to their route of travel.

A roof fall was discovered by the fireboss at approx 1:15 pm. The fall is located in the left return off Lick Fork Mains #3 approx 3,700 feet outby the #002 MMU near SS #8571 at the mouth of the Panel 12 Left. The fall measured approx 20' long x 26' wide x 7-9' thick. No equipment, no injury occurred and no ventilation interruption occurred as a result of the fall.

A roof fall occurred in the 3NSM, #5 entry, #12 crosscut. The fall was 20' long, 30' wide, and 10' high. The fall didn't affect ventilation, travel, damage equipment, or cause injury. The area was bolted with 8' & 10' point anchor bolts.

A roof fall was discovered in the 2 North 2 Mains 107 crosscut between #1 & #2 entries. The fall measures approximately 10' long, 8' wide & 7' thick. The area is supported with 5' fully grouted resin bolts.

The roof failed over and between the existing cribs and rocprops in the tailgate resulting in a blockage. No injuries or damage occurred.

An unplanned roof fall occurred in the #4 entry, left return of old 3 belt. Fall measured 80 ft long, 19 ft wide and 8 1/2 ft high. This area is worked out. No equipment was involved, no personal injuries reported. Ventilation & travelways were not affected. All approaches have been breakered off.

On 10/30/2007 a roof fall was discovered at 12:00pm on the 5th Southwest, entry #1, S.S. 5+10. The fall measured 50' X 18.5' X 7' thick. The area was developed on 4/25/2007, and was bolted with 6' fully grouted bolts. This fall will not be cleaned up.

On 10/29/2007 a roof fall was discovered in # 6 entry of the 2nd North Panel, off the 1st East, S.S. 6+30. The fall measured 120' X 19' X 7' thick, and traveled inby the 6+30 XC. The area was developed on 10/15/2007, and 6' fully grouted bolts were installed. This fall will not be cleaned up.

The slope hoist went down at 11:43 a.m. with a broken gear that runs the Lilly switch. A gear was replaced and the slope hoist was operational at 7:42 p.m.

At approximately 7:20pm in the #10 entry on #2 unit the miner cut into a hole that had water coming out of it. At that time it appeared to be a bore hole. At approximately 9:45pm the section foreman went into the #10 entry and he could smell crude oil. At that time it was determined that the hole must have been an oil well.

Smoke was discovered on the back side of the coal pile at 7:50 a.m. and reported to control room operator who sent a utility person to dig out the smoldering coal. The utility operator, using the loader to dig out and back drag the coal, extinguished the fire at 8:50 a.m.

Our examiners found a roof fall in a worked out area of the 5 left, 1 left, 4 main North panel during their weekly examination. The fall was located at the examiners EP point for this area.

A roof fall was discovered at approximately 1:35 pm on 10-29-07 on the abandon old #1 right section in the #2 entry inby spad 6505. The height and distance of the roof fall cannot be determined due to geology condition.

A ROCK FALL WAS DISCOVERED IN THE #2 ENTRY APPROXIMATELY 70 FEET OUTBY SPAD #533 DURING THE PRESHIFT EXAMINATION AT 8:30 PM. THE FALL WAS APPROX. 20' LONG X 18' WIDE AND 10 FEET IN THICKNESS. THE ESCAPEWAY WAS REROUTED, THE ROCK FALL CLEANED UP AND THE ROOF RESUPPORTED WITH ARCHES. IT DID NOT AFFECT PERSONS, VENTILATION NOR EQUIPMENT.

10-28-07 a fall was discovered outby in track entry adjacent to spad #18483. Fall measured 35' x 19' x 6-8'. Area was breakered off, will be cleaned and additional roof support installed. There was a rider seam 6"" thick, present 8' above the roof. Ventilation was not affected.

A roof fall occurred in the #9 seam in the Main North at crosscut #21 in 1/2 the intersection in #3 entry measuring approximately 18ft. x 10ft. x 6ft. high. The area was supported with 6t. tension rebar bolts and 12 ft. cable bolts. An undetected slip was determined to be the cause of the fall.

South 1, 69 block 8 entry, 4-way intersection, MMU 006-17 ft wide x 35 ft long x 12 ft high. Support: 6 ft 3/4 inch resin bolts, 12 ft cable bolts , pans, brow straps. Fall dangeged off, additional roof support installed around fall. Clean-up plan developed and posted at fall site.

On 10-26-07, an unplanned roof fall occurred on one belt, break 32 between track entry & return entry, spad no. 68314. The return stopping was crushed out. All miners were escorted outby the fall, until stopping is replaced, and the area is deemed safe by the operator.

A power surge from supplier damaged electrical component with production hoist. Investigation showed multiple part failures. Mine evacuated/no problems/no injuries. Replaced 3 SCR's and 1 fuse. Hoist back in service at 1:00am next day.

A BN train delivering limestone to the Coyote Station power plant started 4 spot fires along the railroad tracks. One spot fire was determined to be on the mine permitted area. This fire burned native grasses, but did not threaten any mine structures or equipment. No one was injured fighting the fire. The fire was extinguished in approx. 1 1/2 hours.

On 10/24/07 an unintentional roof fall occurred on the 4th South Sub Main, entry #6, at cross cut # 24. The fall was 55' long x 18' wide x 12' high. The intersection was bolted with 10' point anchor roof bolts. The area was fractured lamanated rock.

Unplanned roof fall above anchorage zone in the 2nd north belt #3 room. Roof fell at spad #3141. The roof fall is 65' long, 20' wide and 7' high. The entry was bolted with 42"" full grouted resin bolts with 6' full grouted resin bolt spotted for extra roof support.

While being pulled onto the ramp to drop into the mine the slope car de-railed and struck a metal pole. Some of the wood planks and metal grating on the ramp were looseded and had to be repaired to provide a walkway. The hoist car was down from 2:20 AM until 4:00 AM.

Tailings pond #823 developed a failure in the eastern berm wall from decant water piping thru a deep linear surface crack into pond #827. No persons or equipment were in danger at any point. Cause is shrinkage of clay material from 3 month drought.

Operator had his bed raised washing material out of got back in truck and failed to lower bed before pulling away and caught the power lines and snapped electric pole. We left operator in machine until power could be terminated.

Our weekly examiners found a roof fall on the 5 West mains at crosscut 36 between entries 1 and 3. The fall was located in an entry adjacent to the examiners route of travel.

A roof fall was discovered on the 3rd MNE, Entry #1, at S.S. 26+25. The fall measured 58' X 19.4' X 12.5'. It was a 3 way intersection, and was developed on 6/30/2007. The area was initially supported with 6' fully grouted bolts, and was later supplemented with 12' point anchor. The fall will did not affect ventilation, or the passage of men. The fall will not be cleaned up.

A rock fall occurred at spad #16145 in the track entry in the old 6A belt area. The fall was 7-10' high, 20' wide and 100' long. The area was bolted with 5' torque tension bolts. The fall will be cleaned up and rebolted.

A rockfall occurred in the belt entry on the bottom between xcut 1+60 and 2+40. The fall was 40 x 19 x 10. The area was pinned with 8' bolts. This is the main north beltline.

Our weekly examiners found a roof fall in a worked out area during their examination. The fall was located om the 3 Left, 1 Left 4 Main North panel. The fall was located in entry #9 at crosscut 31.

The shaft employees noticed a slight deformity in the conveyance cable. Further investigations revealed this to be a minor kink, but repairs would be necessary The mine was subsequently evacuated using the secondary shaft.

Between 12-15 & 10-22 a fall occurred water seal area 2 breaks from EP-W-2A. Fall measured 70' x 19' x 6'. Area was breakered off, will be cleaned and additional roof support installed. Ventilation was not affected.

A roof fall was found at 1:50 AM on 10/22/07. Fall was at crosscut 27 entry 4 in 7LMS. Fall was in four way intersection that had been mined May 2005. Fall measured 20' x 20' x 8'. No injury or property damage occurred.

As the ash truck operator was beginning to dump a load of ash, the load shifted causing the truck to rock back on to the endgate in a wheelstand. The operator was trapped in the cab until a manlift truck could be taken to the location to remove them from the operator's cab. This took in excess of 30 minutes.

Crew was using a cutting torch to remove inaccessable guide bolts; a piece of molten metal inadvertantly fell down the shaft. It is unknown what was smoldering as the shaft is under rehabilitation at this time and it is unsafe to travel down the shaft to extinguish the fire.

On 10/31/07, MSHA determined that a floor heave incident on 10/20/07 was a reportable ""bounce"". Floor heave occurred in the First East Mains #4 Entry Crosscut 14, #3 Entry Crosscut 14 to 15.

D11R Dozer #30 was in shot ripping overburden when ripper came in contact with live shot booster and cap causing an explosion. Damages to the Dozer included a cracked door glass on left side. A nearby 992G Loader #1 received a cracked left side window. NO INJURIES TO EITHER OPERATOR WERE REPORTED.

Unintentional roof fall discovered during examination. Fall measurfed approximately 50'Lx25'Wx10'H. Fall was located at No. 1 section, 11 entry, 17 block, station 4931. Area previously supported with 10' cable bolts and 6' full grout resin bolts. Area developed on or about 8-18-06.

An unintentional roof fall was discovered by mine examiner in old works. Fall measured 20' wide x 350'-400' long x 8-10' high. Fall located northwest mains 1st left from spad #4561 to spad #4554 then to spad #4557.

A roof fall was reported that was an old fall. The investigation revealed this to be an old fall that had rode out the breaker timbers. The fall was located in the old belt entry adjacent to station 7203 and inby station 7228. The fall was breakered off.

A fall was found while a mine examiner was doing weekly examination. The fall occurred in our main west air course entry 0 x-cut 45 sta. 52+25. No property damage occurred and no plans of cleaning fall at this time.

Due to an unknown electrical problem, the ""lily controller"" for the Koepe man cage got out of sequence. Time was required to diagnosis and underground personnel (20) were timely evacuated via production hoist platform. No injuries or problems. Koepe man cage back in service at 7:48 pm.

A tail blockage occurred on the longwall tail gate. The tail was not cutting out to effectively remove coal from the entrance of the tailjack into the entry.

A roof fall occurred in the #11 seam in the 2 East in the intersection of #4 entry at crosscut #6 measuring approximately 25ft. x 18ft. x 5ft. high. The area was supported with 30 inch fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

Hoist outage- the bottom limit switch on the service hoist went to ground. This eliminated automatic of the hoist for 33 minutes until it was returned to service in maunal operation. The mine was evacuated until the hoist was returned to automatic service at apporximately 5:30 am.

On October 5, 2007 excessive smoke was reported on #42 loader in the 5800 14 Stope. A miner was gobbing in the 5800 14 stope on the East side. He was approximately 50' from the gob pile and backing out, when the battery box on #42 2yd loader swung out and struck the left rib. The battery shorted out causing sparks, smoke and what appeared to be fire.

Section Supervisor stated that the Cont. Miner operator was preparing to locate the miner to mine #1 entry. The machine was shut off and they heard the roof working in the intersection. A roof fall occurred in #7 entry of Southwest Mains 12' outby SS#683. The fall measured approx 60'l x 17'w x 2-5'thick. No equipment was involved & no injures occurred. Ventilation was not affected, fall will not be cleaned up.

A roof fall occurred in the 6th East, #6 Entry, at 83 Crosscut. The fall was 20' long, 20' wide, and 9' high. The area was bolted with 8' point anchor bolts. The fall didn't affect ventilation, cause injury, or block travel. It did damage some beltline. No one is required to work or travel in this entry.

An electrician discovered that a roof fall had occurred, in the back entry of the return air course. The area that had caved was approx. 20' long, by 18' wide, by 6.5' high. While drilling a face round in an ore heading, the drill bit intersected an old bootleg that contained a live cap and anfo. The explosives exploded on impact of bit. The jumbo drill was hit with fly rock breaking the windshield and back glass and damaging the cover on the water pump. Called in immediately and investigated next day by MSHA inspector.

Employee was working on a crusher. A bucket tooth was wedged in the crusher. The crusher was jacked up. The jack kicked out and employee was struck in the head by an unknown object and knocked him into the bed of the truck where he struck his head on an unknown object. Employee received head trauma.

The locomotive was coming back into the plant when EE had crossed over to the old scales side road. He got out of his truck & started to cross the tracks when the locomotive operator saw him. They heard him on the radio say he had broke the 7 car. A coworker asked another coworker if he could see him & when he looked back he saw him on the tracks. He had been hit by the locomotive.

Victim was discovered caught between the conveyor belt and the continuous haulage dolley. Although the coroner report is not available, it appeared that the victim expired from injuries received from the event.

Employee was found on the ground in front of a Cat 777D water truck. It appears that the employee fell to the ground from the equipment. There were no witnesses to the event. Autopsy report is pending.

Employee was retrieving pry bar from storage area on the ground, when personnel above him lost control of new return roller they were changing out. The return roller fell, striking the employee on the ground. While the employee was wearing proper safety equipment at the time, the impact of the roller resulted in his death.

The operation was crushing recycle concrete and asphalt 40% and mixing it with a 60% sand-gravel mixture. With a little wet clay in the sand-gravel mix, the crusher jaws plugged up. Roof bolting with Fletcher RR2 twin head bolter, he had 4' resin bolt in hole and it was partially bent. He was pushing on bottom end of roof bolt with his body weight trying to straighten bolt when bolt twisted in hole causing him to fall into the bolt hurting his shoulder area.

After bolting 5 left x-cut ee walk to the back of bolter and was talking to the section foreman then as he was walking back to the front of bolter a rock fell out an hitting him on the back an dleft leg. The size of the rock was 4 foot by 3 foot by 3 inch thick.

The truck and cab slid backward down the face of both sides of the highwall corner about 50'. The injured was wearing his seat belt and suffered a cut on his head requiring 3 staples to close it. Employee was removing weld from the rodent screens of the motor to gain access to the inside of the motor from cleaning. The employee was using a hammer and chisel to dremove the weld. When he hit the chisel it broke off a piece of welding slag that struck his right eye. EE had been using safety glasses, however he removed them due to fogging & continued to work.

The man was kneeling down and started to bleed the control transformer to ground when he got shock.

On 11-21-07 employee brought to the attention of the Safety Dept. he had missed a day of work due to a work-related injury he received 10-27-07. He was struck by a piece of draw rock while bolting top. He finished his shift, went to a local E.R. the next day (Sunday) and was instructed by the E.R. physician to take the next day off work.

Employee was being loaded with lime at the south lime silo when he noticed dust in his cab. First his nose and sinuses began to burn and then his mouth, throat, chest, and eyes. He was taken to NNRH and held for observation.

As they were standing there a van driven by another TBCC employee began backing up towards the truck. Employee stated that he had installed a bolt close to the rib and had let the pressure off his canopy to swing his boom over toward the center. The canopy didn't come down, it was hung on the rib so he released his floor jack to swing the boom over, not realizing what could happen. Our employee was traveling in a ""jon"" boat out to a dredge. The boat had a small outboard motor attached to it. The throttle on the engine malfunctioned and the boat did not decelerate as anticipated. The boat then struck a ladder attached to the dredge. Upon impact, our employee fell forward, striking his knee on the deck.

Employee had switch trucks earlier in shift and was returning to the first truck to pick-up something he had forgot. Both trucks were CAT 785B Rock Trucks. He was taken to a local hospital, examined & transferred to another hospital for surgery.

WHEN THE PLATE WAS HOISTED TO THE BUCKET, THE CRIMPED END OF THE CABLE GOT STUCK IN THE HOLE. BILL THEN TOOK A SIX FOOT PRY BAR AND CLEARED IT. THE SUDDEN RELEASE OF ENERGY FROM THE BAR PULLED THE SLATE BAR TO THE BUCKET, SMASHING HIS LEFT THUMB.

EE worked the remainder of his shift and worked the next day and then went to the Doctor. The Doctor released him to return to work but ordered an MRI be taken. After the MRI on 11/14/2007 the EE underwent surgery on his left knee.

EE was grinding on the #14 conveyor frame when a piece of steel flew off & entered his eye. At the time he thought he just got some dust in his eye & cont. to finish the work day. He worked the entire next day & woke up in the middle of the night w/eye pain. On the 17th he called in & went to the eye dr where they dug a piece of steel out of his eye.

Pulling screen panel up the screen, EE didn't get his finger out of the panel in time and it was jammed between the screen panel and a spray bar. His middle finger was cut and required medical attention. He returned to work the following day. 6 sutures.

Employee was blasting short holes for sill timber at raise prep. Loaded 2 holes with 2 sticks & 1 fuse primer each. 1 shot went off - employee thought both had gone off. Re-entered blast area almost immediately, then 2nd hole went off peppering him with rock.

A non-fatal excavator accident occurred on 10/12/07 when the Hitachi 450 Excavator was clearing trees from below and above the high wall. While attempting to push them down, the excavator came too close to the edge and slipped over the high wall.

Haul truck driver complained his back hurt at end of shift on Thursday 10/11/07. Driver did work Friday as a labor but not on mine property. Sat. driver went to doctor and driver reported to employer on MONDAY that he would be off this week due to back. On Friday 10/19 driver reported he would return Monday 10/22/07. OFF work 5 days.

The employee alleges that as he was building a ventilation brattice he picked up a cap wedge and raised it over his head to wedge the brattice in place. He alleges that he felt a pop in his right shoulder as he lifted the wedge over his head.

Trailer bed was raised to disconnect it and when it was lowered again, employee had his hand on the trailer brace and his thumb was crushed.

Employee was using a hammer to tighten cheek plates in jaw, when eye that was holding cheek plate broke off, striking employee in lower right arm, causing a bruise.

The coal truck was coming down the hill and met another truck coming up the hill. The driver got over too far and ran into the ditch causing the truck to turn over against the bank. As the driver was pulling himself out of the truck, he hurt his shoulder.

Bolter operators were loading 13' Long metal straps(used for roof support). While turning to load more straps on the bolter the victim's arm hit the corner of the other straps being loaded lacerating the victim's left arm in the elbow area.

Employee was moving large stone to split when the stone slipped, catching his ring finger on his left hand, between 2 stones.

Miner was taking a tie-rod end off of tractor. Heating the tie-rod end caused the ball to come out of the socket under pressure striking the left index finger and right lower leg.

Injured employee was positioned at the end of the line curtain in no. 3 heading when he stepped into the path of a shuttle car that was in the process of backing up, pinning him between rib and shuttle car, resulting in a contusion and laceeration to his left leg.
