An employee was loaded and headed east going to the scalehouse when he saw a train on the south end of Old Hole, and said he looked to his left and R50 Euclid was too close to the crossing and he did not have time to stop.
A roof fall occurred in the 2 West 3 Mains, #6 entry, crosscut 16. The fall measures approximately 18 feet long, 18 feet wide, and 6 feet thick. The area is supported with 5 foot fully grouted resin bolts and 10 foot point anchor bolts.
An employee became stuck on the Hallam Portal elevator upon entry into the mine. The elevator stopped about 70 ft. down from the surface landing. The elevator service contractor was immediately called and was able to get the elevator back in service by 2:45 AM. There were no injuries as a result of this incident.
A roof fall two breaks inby spad # 9298 from #5 through # 6 entries occurred in panel 5 off of the Panel 1 Submains (005 MMU).
An unintentional roof fall occured in the old 2C panel return at station 72+95 entry #6. The fall did not affect ventilation, passage of men or cause pesonal injury. The fall measured 20'x20'x6'high. It will not be cleaned up.
A roof fall occurred at 13th East Gateroad, Unit 1, Crosscut 69, Entry 3. The fall measured approximately 8' long x 16' wide by 4'-6' thick. The fall did not result in any damage to equipment or property.
Hoist was down due to the key steel stripped out of the reducer.
During the weekly examination, a reportable roof fall was found in E South Mains, entry 7, at crosscut 28. The fall was approximately 40' in length, 20' wide, and 10' high.
A roofall occurred one break inby spad # 9317 in the #5 entry in Panel 5 off of Panel 1 Submains (005 MMU).

A roof fall occurred in the #9 seam in the 2 North Panel off the 2 Main East in #10 entry between crosscut #39 and crosscut #40 measuring 30 ft. x 18 ft. x 5 ft. high. This area was supported with 5 ft. fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
Location of fall: Rt. return: Entry No.9; Spad No.9965; Distance from active face: 1200 ft. No equip involved; Airway: Neutral; Size of fall: L - 60' H - 19' W - 4.5' Strata: Shale; No coal rider seam present; No cribs or timbers installed prior to fall; Area will not be rehabilitated.
There was an unintentional roof fall on North Mains 39 x-cut inbetween #5 & #6 entries. This is an inactive area and travel is not regularly required. The area measures 30' long, 18' wide, 5' high.
Hoist was down due to a stripped coupling and shaft.
An unintentional roof fall occurred on the Galatia North #5 belt, #2 entry between crosscuts 155 and 156, approx. 13875' north. The fall was approx. 60 ft long, approx. 19 ft wise, and approx. 12 ft thick. The fall did not affect ventilation or the passage of men. The fall will be cleaned up.
The discharge screen deck on #6 tertiary crusher caught on fire during a plant startup after a scheduled fine crusher shutdown. After an initial attempt to put out the fire, the local fire department was called and the fire was out by 5:30 pm. No injuries were sustained.
A roof fall occurred in the 2 west 1 mains crosscut 67, between #2 & #3 entries. The fall measures approx 30 feet long, 15 feet wide and 6 feet thick. The area is supported with 6 foot fully grouted resin bolts.
7 Mains 2, 7 Entry, 26 Block, MMU 002. 25 ft wide x 25 ft long x 8 ft high. Support: 6 ft 3/4 inch resin bolts, pans, brow straps, 12 ft cable bolts. Fall dangered off, additional roof support installed around fall. Fall clean-up plan posted.
Primary crusher was overloaded and seized while 350 h.p. drive motor continued to run causing the drive belt to catch on fire. MSHA was called at 7:20 pm; fire was out by 7:25 p.m.
The mine experienced an unintentional roof fall outby the active section on the parallel leatherwood mains at crosscut #45. The fall is approximately 160' long & 5' to 6' thick. The roof in the area is layered shale. This area had been permanently supported with 4' fully grouted rods. The fall extended thru the intersection from #3 to #6 entries.
A roof fall occurred in the 6 ep, #74 crosscut, #10 entry. The fall was 18' wide, 18' long & 9 high. The area was bolted with 8' point anchor bolts. The fall didn't affect ventilation, block travel, damage equipment, or cause any injury.
A roof fall was discovered near the Lambert Branch intake shaft by the fireboss as he was conducting his daily inspection.
Employee was entrapped on the elevator when the safety switch on the travel rope tripped due to the elevator being bounced from it being jogged manually in the inspection mode. Schindler elevator inspected and reset the safety to allow employee to be removed from the elevator. He got off the cage at 12:54 AM
An unintentional fall of the roof on the 6 left section at spad #288. The fall was 2000' from the working section and measured 60' long, 18' wide & 6' high.
Fall happened in crosscut of #4-#5 in first southeast section seven crosscuts outby face. Fall was 30 ft long, 18 ft wide & 42"" high.
An unintentional roof fall occured in the 1A panel returns at station 29+40 entry #10. The fall did not affect ventilation, passage of men or cause pesonal injury. The fall measured 25'x20'x4.5' high. It will not be cleaned up.
Approx 6 pm on 11/7/07 a roof fall was discovered during. The preshft exam. The fall was in #1 entry, between breaks #16 & 17 off the #1 belt line. The fall measured 60' long x 18' wide x 5' high.
Unintentional roof fall occurred 10000' outby working section 40' long x 18' wide x 6' high. Between #1 & #2 entry at spad #480 2nd Northwest Mains. Fell on top of service bolter; cribbed off, will not be recovered at this time. Found during weekly exam.
Unintentional roof fall occured at White Buck No. 2 Mine. The fall occured on 1st Right off of the Southwest Mains at Spad No 10141. The fall measured 10' length, 20' Wide, 6' in Height. No injuries occured, no passage of men affected.
A roof fall occurred at 1st North Main East, Crosscut 56, Entry 6. The fall measured approximately 22' long x 24' wide x greater than 9' thick. The fall did not result in any damage to equipment or property.
The Cedar Creek Elevator was out of service from approx 1:00 am until approx 4:30 am due to locking mechanism.
An unintentional fall occurred in 4th north panel, #10 entry at the 11+30'. The fall was 20' X 20' X 7'.
A 25' x 20 x 8' thick roof fall occurred in No. 2 entry, one break inby spad No. 15846. The area will be breakered off and air will be rerouted into the no. 1 entry.
An unintentional roof fall occurred on crosscut 125 at the galatia north portal. The fall was approx. 40' in length, approx. 17' in width and approx. 8' in thickness. the fall did not affect ventilation or passage of men. The fall will not be cleaned up.
After initial inspection and tests were run, hoist did not respond properly. Computer was reading a PLC communication fault. Technicians were called out, and at 9:45 AM the hoist responded. Following inspections and tests, the hoist was back in service.
A Non injury unplanned roof fall occurred in the bleeder entries of MMU 004/005 working sections.
Explosive misfire: drill operator, while drilling Co. Rd. mine, heading 14E & 75S, machine drill steel struck an unexploded explosive, consisting of a cap, stick of dynamite and small amount of anfo. No damage to machine or operator.
The roof fall was 40' long x 18' wide x 4-7' thick in the laminated slate roof. The fall was in the belt entry on #7 belt at spad #2602. The fall was cleaned up and re-bolted according to the Rehabilitation Plan. The fall was approximately 500' from the active section.
At 10:40 AM the hoist stopped responding and all men were immediately brought up out of the mine. Technicians were called out, and problem was found to be a bad wire connection in an electrical panel. The connection was repaired, and after operating the hoist in test mode for 15 minutes, the hoist was again operational.
Our examiners found a roof fall during a pre-shift examination of a worked out area. The fall was located in the 2 right, 1 left, 4 West panel from entry 3 to 5 at crosscut 11. The fall was adjacent to the roadway where the examiners were traveling.
A ROOF FALL WAS LOCATED BY MINE FOREMAN ON 11/24/07. HE IMMEDIATELY NOTIFIED THE OUTSIDE PERSONNEL WHO NOTIFIED MSHA. THE FALL OCCURRED THREE BREAKS INBY SURVEY STATION # 1273 IN THE #8 ENTRY. Discovered a small roof fall in x-cut 12 between 1 & 2 entry in the tailgate of LW-202. The area was dangered off and additional supplemental supports were installed to prevent the fall from growing. The fall was approximately 12 ft long and was rib to rib at the mouth of the crosscut toward number 2 entry. The fall was above the roof bolts.
Mine elevator stopped while transporting person. Computer system was reset and person was removed from cage at 8:50 AM.
At the 5-5200 south bench, a fall of ground from the back fell approximate size 10' by 10' by 20' long. The area was not open to personnel only remote equipment. The fall was approximately 178 feet from where personnel had access. A miner thought the rib had failed and reported it. The next morning the mine foreman went to look it over and determined the failure was from the back.
Our purchased power supplier had an arrestor blow up at an off site location causing a power outage at our location.

A roof fall was found at 3:40 am on 11/17/07 at cross cut 74 entry 5 in 5 rms. Fall was 8' high, 24' wide, and 20' long. Area was mined 10/24/04 and had been bolted with 8' full resin pins. No injury or property damage resulted. There are no plans to clean up fall.
A 10' x 19' x 6' high roof fall occurred in No. 1 entry approximately 150 feet inby the return air shaft, 3200 feet from No. 1 Section. Roof is supported with 48"" resin bolts. 2 cribs were installed prior to the fall. All entries to the fall are breakered off.
A maintenance man was air arcing some bit holders off of a borer miner and the sparks ignited a methane seep in the floor. The maintenance man kicked dirt on the seep putting it out. The borer miner was located outby the last open crosscut.
An ignition occurred at approximately 98+94 in #3 Entry, E-17 Section. The ignition lasted 1 to 2 seconds and was self extinguished by the water sprays from the continuous miner. There was no damage to equipment and there were no injuries.
Roof fall above anchorage in 22-Right Belt line, # 3 entry, XC 87+50 extending to XC 88 approximately 115 feet in length and 22-feet in width. This panel was mined July 2007 and it is believed that the water, heat and humidity along with the dry winter air now in progess may be a contributing factor of changes in the roof conditions.
Lost Allegheny Power at #7 Substation at approx 5:19pm. The diesel backup failed to start and the mine was evacuated. Power was restored to the #7 Substation at 6:51pm. The #7 fan was started on electric and the power then tripped. During that outage, the #8 elevator lost power. The power was restored on the elevator and after a round trip it was returned to service.
Hi Wall fall, on SHM Miner #17. Approximate size of fall was 60' high, 20' wide and 125' long. No employees were involved. Employee was doing maintenance on miner at the time. Miner was backed away fron the Hi Wall at the time of the fall.
on 11-16-07, around 6:30 am a roof fall was discoved at number 9 break on number 1 belt line. The fall measure 4'x18'x18'. no injuries were reported as a result of this roof fall.
Employee was racking out motor switch gear for fan # 71FA07 at MCC J. He checked with the control room and asked if it was okay to lock it out, but failed to check the guages to verify the switch gear was not under power. The employee disengaged a 4160 volt switch under load causing an arc flash and destroying the breaker. The employee was not injured.

As water receded in the old Northwest Mains area during pumping efforts a roof fall was identified in the #7 entry near spad #3737. The fall was approx. 120' long X 20' wide and 4' high. The area contained 4' resin and 4' dbl. lock bolts. The area was mined approx. 11/93. The area has been dangered off and timbers have been set.
Fall was in the neutral entry between 2-3. Spad 6379. Fall was 40' L - 20' W - 8' to 9' H. It was bolted with 6' torque tension fully grouted bolts, developed 4-15-07. Area will not be cleaned up and no injuries were recorded. Airways and escapeways are not affected.
While mining crosscut 5-4 at 3 + 60 25' into the crosscut an uncharted 9 5/8 drill hole which contained H20 & CH4. The power was de-energized, reventilated and deluted below 2%. By 11:40AM power was restored and pumping VIA 2 8 hp pumps out 1-4"" discharge 1- "" discharge line.

The fall was approx. 80' long X 20' wide and approx. 5' high. The area contained 4' glue and 4' dbl. lock bolts. The area was mined in 1993. The area has been dangered off and timbers have been set.
While making examinations discoverd a rock fall in the inactive work areas in the # 1, # 2 and # 3 entries at the last open crosscut in the right return of the 004 and 005 section. The fall varied in height from 1 to 6 feet for approximately 130 feet. No employees were injured.
At spad # 22139, 1260 feet from the face #1 south west sub main rock fell. 5' high, 18' wide, 25' long. Fall will be dangered off and supported as per roof control plan.

An Un-Intentional roof fall was found in the number 5 entry outby spad # 651. The size of the fall measured 60 feet long by 20 feet wide by 5 feet high. All approaches to the fall were cribbed and dangered off. The fall occurred due to weathering in the mine roof. Type of roof support installed was 4 foot double-lock resin assisted bolts.
the hoist was out of operation (from approx. 850 am until 952 am) due to an electrical power supply outage. operation resumed after the utility company restored the electrical power.

MMU 001-0, #9 entry. While cutting coal a gas or dust ignition occured. The flame traveled outby the miner head approximately 15 feet and then extinguished. There were no injuries. There were several kettle bottoms in the top at this location.
Rock fall occurred on 1 North Mains beginning at break 5 and extending to break 8 in #6 entry in the intake. Fall measured approx. 19' wide x 270' long x 5'-6' thick.
An unintentional roof fall occurred at 39 x-cut 10 left track entry. The area is inby active workings and is used only for access to Bleeder evaluation points and seals. Two travelways remain open to the area.
An unplanned roof fall occurred in the #3 belt entry of Southwest Mains just outby the face. Fall was approximately 7' high, 18' wide and 45' long. The affected area was bolted with 6' tension roof bolts according to the approved roof control plan. Did not affect any ventilation or equipment. Fall will be cleaned up.
The electrical windings in the rotor shorted in the #1 hoist motor. The mine was evacuated at 5:45 a.m. The hoist motor was replaced with a new rebuilt motor and the hoist was tested. Miners returned underground at 1:45 p.m.
Our examiners found a roof fall in a worked out area during their weekly examination of the area. The fall was located in the 2 left, 3 left, 1 left, 4 main North panel in entry #1, crosscut 9. It was located in an area near one of their examination points.

Foreman doing his preshift found a roof fall in last intersection in belt line fall was about 30x30 intersections was cribs up and danger off.

A linkage that connects the hoist brake electric coil came loose causing the hoist brakes to lock up. This caused the hoist to trip on over current.
An unintentional fall of roof occured in #5 entry, cross cut #33, on the 4th South Sub Main. The fall measured 20' long x 18' wide x 15' thick. The fall consisted of fractured lamanated stack rock. The area was bolted with 10' point anchor bolts. The fall will be cleaned up and resupported.
The tailgate traveway was blocked to floor level and additional roof support in the #3 entry of lowest inby spad #35 to 00 between the #21 and 22 X-cut for a distance of 200'. No one was injured and the section went under our blockage plan.
an unintentional roof fall was found on the 6 left section #4 entry at spad #457. The fall is 2000' from any active workings. The fall measured 45' long, 18 ft wide, and 8 ft high.
A roof fall was discovered in an old panel that the #4 section has moved out of. No injuries or disruptions to ventilation resulted. Location - 3rd Panel So. off 1st Panel So. off Main East @ Spad 21+28. Approx. dimensions - 75' x 20' x 8' high. This is an old panel so there are no plans to load the fall out.
Fall was 100' L, 18' W & 4'H located in an abandoned area 10,000' from the nearest face. Area has to be traveled once weekly because it is a return. Clean-up plans posted. No injuries or air was affected.

An unplanned ignition of coal dust occurred in the panels to open and vent pressure. The ignition of coal dust caused the explosion vent panels to open and vent pressure. There were no personnel or equipment damages. The investigation as to the cause of this event is ongoing.

Operator smelled smoke and exited machine to investigate. Found fire in the battery compartment and tried to put out with extinguisher. Fire would not go out and operator initiated mine evacuation.
The apparent failure of a tail roller bearing resulted in the misalignment of the No.6 belt conveyor and caused belt conveyor material to wrap around the shaft of the tail roller. This caused a belt fire accident at the tail roller of No. 6 belt conveyor in the draw-off tunnel on the surface.
EE was charging on 103EPL using 0284 charging rig. While charging holes, EE noticed smoke coming from the comprssor. EE shut off rig and turned off master switch. Fire was too big, so he left area to go page phone to call foreman. Both Buick and Casteel mines were evacuated. MSHA was called and notified of fire within 5 minutes.
An unplanned roof fall occurred in the #130 mine in the old #3 panel on the #1 section 1 brk. inby spad # 7607. The fall measured 125' long, 6' high and 20' in width. It was approximately 1200' from the nearest section. No injuries were reported. The area was developed on 8/29/07.
An unintentional fall occurred in the 3rd north old panel. Fall fell from #1 entry over into #2 entry at the 32+00'. The fall was 70' x 19' x 8'.
A roof fall occurred in the 2 North 1 Mains, 14 Crosscut, #4 entry. The fall measures approximately 30 feet long, 18 feet wide, and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts, 6x6 timbers, and 12 foot cable bolts.
Backfill jammer operator was inadvertently undercutting nose pillar while jamming backfill; haul truck operators delivering backfill to stope were damaging ground support in high rib while backing into stope. Combination of undercutting of low rib and ground support damage to high rib resulted in rib failure.
Fall was discovered in the No. 6 intake entry of No. 1 North at survey station No. 563. The fall measured approximately 20' long x 20' wide x 6' high and will not be cleaned up. Ventilation was not affected. Passage of men was not affected. There are no injuries or equipment involved.
An unplanned roof fall occurred in the #130 mine inby old # 5 tail, bkr. 46 to 47 in the #4 entry. The fall measured 20' long, 18' wide and 6' in height. No injuries were reported. The area was developed on 6/3/06.
An unintentional fall occurred in the #7 entry on the 4th north panel at the 17+90'. The fall was 60' X 19' X 7'.
The main hoist went down at 10:45 am with a broken soe on the shaft guides. The shoe was replaced and the hoist was operational 1:55 p.m.
An unintentional roof fall occurred approx one crosscut inby SS# 3779 in the #4 entry of the Mains section. The fall dimensions were approx 20'W x 45'L x 6'H.
Hoist went into overspeed conditions due to software fault. MSHA notified via Immediately Reportable Hot Line.
elevator down due to control computer malfunction. Computer rebooted to reset system. Running at 11:00 PM. No entrapment
The belt transformer at the No. 3 Belt Drive caught on fire. The CO system sounded an alarm and the section crew was notified. The crew was brought down the intake outby the fire & some of the crew members put the fire out with fire extinguishers and water after it was determined there was no power on the transformer.
An unintentional rock fall occured in the 1A panel at crosscut #46 between entries 7 and 8. The fall did not affect ventilation, passege of men or cause personal injury. The fall measured 25' x 18' x 41/2' high. It will not be cleaned up. It had been supported with 48"" fully grouted roof bolts.
Roof fall 1st east panel #2 unit x-cut 32 #2 entry. 85' long x 6' high. Developed Feb'07. phoned in to 1-800 11/5/07.
Roof fall was discovered on E-mains #6 entry @ #9 cross-cut spad #80. Outby section and loading points. Developed Sept. 2005. 190' coverage. Area bolted with 6' double lock bolts, 7/8"" dia. grade 75, 8x8 inch roof plates with roof screen installed. Fall measured 25' long, 20' wide x 9' high. No injuries or equipment involved. No ventilation or escapeway blockage.
A rock fall was discovered on 11/5/07. The fall was located in roadway going to bleeder. The rock fall was approx 90' long in all 3 entries. The fall was from 3' to 8' high.
A roof fall occurred on the 3rd west belt 3 xcuts outby room #1. The fall was 35' long, 35' wide & 10' tall. It did not effect ventilation, cause injury or block travel.
A fire occurred at 15 corsscut of #1 5 south belt conveyor. Coal was smoldering at a bottom roller on top of an overcast.
An unintentional fall occurred in the #10 entry of the 4th north panel at 10+10'. The fall was 19'6"" X 19'6"" X 10'.
An unintentional roof fall occurred in the intersection of the South West Submain in entry # 4, # 32 crosscut. The cause of the roof fall was laminated and unconsolidated shale at the anchorage point. The area had been supported with 6 foot fully grouted roof bolts.
A fall ov the roof occurred on the #3 unit, belt entry,1st NW sub mains, the fall measured 40' L, 19' W and 5' HT. No ventilation, equipment or personnel was affected.
The electrical power was interrupted to the #8 fan for a period of time exceeding 15 minutes. All individuals were evacuated according to law. The power was restored and a special exam was conducted and the mine found safe for re-enter 3:35 PM.
Fall was found by the preshift examiner on the 3rd shift it was located two cross cuts inby survey station 12770 in the number five intersection catching the head of the continous miner. Fall is approx. 20'x20'x6' high it was investigated by MSHA and OMSL. Fall will not be cleaned at this time the section will be moved outby this bad area.
A roof fall occurred on #1 unit, #1 entry, from 15 to 16 cross cut. This section is in the 2 main west #3 mains. The fall measures approximately 70 feet long, 18 feet wide and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts and 10 foot point anchor bolts.
A roof fall occurred at Main East Travelway, Entry #4, Crosscut 156. The fall measured approximately 20' long x 20' wide x greater than 14' thick. The fall did not result in any damage to equipment or property, but did affect passage of miners.
A rock fall was found 30 feet inby spad number 295 in the number 3 entry of the number 1 mains. 20'W X 20'L X 5'H. No injuries occurred in this rock fall.
6 mains, 21 block, #2 entry, 4-way intersection, MMU 007-17ft wide x 30ft long x 7ft high-Support: 6ft 3/4 inch resin bolts, pans, brow straps, 12 ft cable bolts-Fall dangeged off, additional roof support installed around fall-Fall clean-up plan posted.
Fall on #4 2nd belt (#6 entry x-cut 12). Length 40' x width 40' x thickness 8'.
A roof fall occurred in the number 2 entry of the 3rd north mains between crosscut 91 and 92. the fall was approx. 5.5 feet high X 20 feet wide and 75 feet long. the area was supported with 6 foot resin bolts during initial development.
A bounce occurred on the First East Mains section with floor heave and rib sloughage in several areas across the section.
While welding a restraining clamp on the #2 shield of the 8O longwall an orange flame was observed on the mine floor. Employees in the area quickly put the flame out with a combination of a fire extinguisher and a water hose. Both of which were located at the site.
A hot spot was found in the 5-right intake air course #7 entry 5 X-cut sta. 6+00 at 3:30 pm on 11/3/2007. The hot was found at a fall area. We were instructed to try and load fall to reach the burning material. This was unsucessful and on Tuesday, 11/6/07 at 3:00 pm, permission was granted to build 4 seals around the area.
Roof fall discovered on the North Mains, outby section & loading points. Developement date: Nov. 2006, 300 ft. coverage. Area bolted with 6 ft double lock bolts - 7/8"" dia, grade 75, 8x8 inch roof plates with roof mat installed. Fall: 16 ft wide x 16 ft long x 10 ft high. No injuries, no equip involved, no ventilation or escapeway blockage. Notification made within 15 minutes.
Elevator inoperable for more than 30 minutes. Door switch on the bottom malfuntioned. No injuries involved. No one trapped. Steps to prevent. Continue daily maintenance and checks.
Undetected hot lime material from #6 kiln cooler system caught the stacked long tunnel conveyor belts on fire. Hand extinguishers could not control the fire. Local fire dept was called @ 12:35 pm and began applying foam suppression after 12:50 pm. Fire was controlled and out by 1:20 pm. MSHA called @ 12:56 pm (SR #1-48113790). There were no injuries involved.
An unplanned roof fall occurred in the #2 Gas mine that measured 45' long, 18' wide and 10' in height. No injuries were reported. The area was developed on 4/27/07.
2nd shift 220 feet in mining hit auger hole. Mines pre law. No accurate mine maps are available. Inundation of water.
A methane gas ignition occurred on the #1 section. The miner was starting a new cut in the # 3 right face, As the miner was cleaning up the coal accumulation on the exhaust curtain side a ignition occured in the bottom area.
Unintentional fall occurred in face area of #2 working section MMU-003. Fall occurred due to fact that rock broke above anchor point. A cont. mining machine & load end of ram car were covered up. No bodily injury or damage to property. Entrances to area will be reinforced, rock cleaned off machinery, & machinery recovered. All adjoining areas will be inspected prior to recovery.
The mine encountered a roof fall on the Main South beltline (entry #4) at S.S. 6+60. The fall measured 50' X 18' X 8.5' thick. The fall affected production on MMU-002. The 800 number was called. The area was bolted with 8' point anchor bolts.
There was a roof fall discovered 4000' out-by the section in the return entry. It is unknown of the time of occurrence. This area will not be rehabilitated. Has been dangered off and timbered off to prevent any access.
A fall was found by MSHA inspector in the #9 entry of the 3rd panel south #4 unit at spad 21+51. Approx. 120' long x 20' wide x 10' high.
An unplanned roof fall at the anchorage zone in active workings where roof bolts are used. The fall was approximately 15' long, 15' wide and 6' high.
The water level in the settling pond increased higher than past years and washed out over the top of the berm releasing approximately three feet of water from the pond, leaving the ground level at 4"" - 6"" deep in the immediate area. No employees working in area. New overflow pipe installed and the dike has been repaired.
An unplanned roof fall occurred at spad #2485. Fall was in the #4 and #5 entry. Ventilation was not altered, passage of persons was not altered. All entrances to fall were breakered off. Area will not be rehabilitated. Fall measured length 50', width 20', height 6'.
Roof fall on the main west section F & G entries at 45-47 F entry to 45-79 O entry. Fall was approx 100' long X 17' wide X 7' high. Shale and sand stone top.
A roof fall occurred on #1 unit, mmu-001-0, 16 crosscut, #10 entry. The fall measures approx. 15' long, 10' wide & 6' thick. The area is supported with 5' fully grouted resin bolts and 10' point anchor bolts.
A roof fall occurred in the #11 seam in the 2 East in the intersection of crosscut #5 in #7 entry extending through the pillar line approximately 15 feet towards #6 entry meauring approximately 50ft. x 18ft. x 6ft. high. This area was supported with thirty (30) inch fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
Employee was using elevator to change levels, an extension cord that he was carrying caught in the elevator door disrupting the operation of the elevator at 8 ft below the initial level. Employee contacted his coordinator to report the stop. Elevator vendor was notified and MSHA was contacted immediately. Elevator vendor arrived and was freed employee in 40 minutes.
While dumping short for a dozer to extend higher berms, the injured backed his end dump truck into a different area away from the dump area. The victim landed upside down in water at the bottom of the pit. The victim was pronounced dead at the scene by the medical examiner.
Conveyor Belt broke free from feeder breaker and struck employee on his back, knocking him down.
While The Page dragline was still digging, the Assistant on the dragline crew entered the area in the back of the dragline w/o first notifying the dragline operator. The Assistant was injured when he was struck by the dragline shoe as the dragline turned to deposit a load of material. The Assistant was promptly transported to the hospital and he died there the following day.
Employee was alone on the 3rd floor of the plant when he fell approximately 20 feet to the ground floor.
An oversized rock in the feed material became lodged on the conveyor. The conveyor was locked out and the miner attempted to remove the rock. The rock moved and landed on his finger. When he pulled back his finger, the tip of the finger was severed.
EE was struck by the 488 scoop and was caught between the scoop and the 21 S/C at or around spad #580, breaking his left leg.
After loading a tank truck, the employee was retracting the loading spout from the truck . While swinging the loading spout from the truck at the pivoting point , the tip of his right ring finger was severed in the pinch point where the spout pivots.
Employee never reported an accident or injury. He had stomach pain, went to personal doctor, doctor determined employee had a hernia and informed employee it happened at work. Accident date listed above is date of doctor visit. Currently working with work restrictions.
The alleged injury occurred when the EE was installing a new cable for the quarry pump. The EE was pulling on the cable, when he felt a strain, resulting in a hernia.
Was placing bag in tilt chamber, chamber cycled & finger was caught between rake & rolled edge of tilt chamber. Amputation of first joint, ring finger, left hand. He has restricted use of left hand. No lost time.
The powerscreen was being raised and EE had ahold of the stabiilizer leg while the machine was being raised with the hydraulic control. The hydraulic hose blew and the machine fell. EE's finger got pinched between the stabilizer leg and the main frame. It was amputated between the 1st and 2nd joint of the left middle finger.
Tech was exiting the house of 325 overburden shovel. The tech lost balance and grabbed the door jam to keep from falling at the same time the door slammed shut on the ring finger of the left hand amputating approximately one quarter inch of the finger.
Employee was assigned to clean 3rd deck of preheater area. While operating the fork lift, the employee placed left hand between the hopper and the forks which caused the dump part to kick back and severed his index finger (L) below the 1st joint area.
Employee's left arm was caught in the tailpiece and dimembered from his self.
Employee was cleaning out lumps of clay from a rotary valve; the employee stuck his hand in the rotary valve, while it was running, and lost the very tip of his third finger on his right hand.
While attempting to move an adapter for the 994 Loader that fell of off a Service truck, the adapter slipped from the hands of the employee pinning his 2nd finger of left hand between the adapter and railroad track. The resultant injury was an amputation of the 1st joint of that finger.
Employee was looking up into the glory hole, from below, to determine why it was plugged. A small amount of crude fell down in between the employee's safety glasses and his face, and scratched his left eye.
Oiling gearbox for cooling tower fan in GR3, he fell from a ladder straining his right arm and shoulder. Restricted duty: no overtime.
Replacing cover plate on miner, the lid fell pinching his ring and middle finger on his right hand.
Right break #7 entry, 2 lines outby face. Bolting ribs in belt channel, after putting bolt in rib, he went to lower boom, hose busted, spraying him in face and eyes.
Injured was bringing drill steel out of hole when he bumped rotation button causing steel to come out and hit him on left arm.
Employee tripped over a chain attached to a come-a-long and fell onto a rock with his knee.
End dump driver was backing under pit loader, pit loader said his bucket dropped, hitting bed of truck and causing haul truck to come off ground. When pit loader raised bucket up, haul truck came down, causing it to jump forward causing employee's neck to move suddenly. Haul truck driver was wearing seat belt.
While cleaning out the bypass chute behind crusher 2, employee stepped on section of the catwalk that appeared to be rusted and fell partly through hitting his right thigh on the electrical conduits and the tail pulley guard. As a result employee sustained abrasions on forearm, contustion of the thigh and laceration to left palm.
Employee stated he was operating hauler on road from pit to dump area. He hit a dip in road, causing hauler to bounce up and down onto road. Jarred employee. Not reportable till lost work on 12-3-2007.
While loading bolts in tray on roof bolter, EE felt pain in lower back.
EE was stepping up on the boarding ladder. As he began climbing up on the ladder, he felt a very sharp pain shoot up his lower back. The pain progressed when he started working, causing him to leave early.
While injured was tramming bolter from one work place to the next work place, he put his arm outside the tram canopy deck area and trammed bolter into rib causing arm to get caught between rib and bolt machine.
2nd North trail, firebossing, walking around and turned right ankle on rock.
The employee was working on the section conveyor belt tailpiece when his wrench slipped. When the wrench slipped it caused him to strike his elbow against the tailpiece. He worked his regular job until 2-26-08. He did not miss work or receive medical treatment until 2-26-08.
He stepped on a rock and twisted his ankle. Missed work 11/30 & 12/3. Returned to work on 12/4.
The employee stepped of a Ditchwitch 8020 rock saw and twisted his ankle when he touched the ground. EE was off work until 12-3-2007. When he came back, staff looked at his ankle and directed him to go to the emergency clinic right away. The ankle was x-rayed and found to be broken. EE was casted and sent home.
Loading a box of resin on the roof bolter, pulled something in right shoulder.
Employee was operating the #553 haul truck and slid into the back of the #518 haul truck.
EE was operating an automated polishing line. The hose which supplies water to it came off. He cut off all electrical, then went to back of machine to cut off water source where he slipped and knee buckled causing fall.
Emp allegedly reached with left hand to grasp handle to pull himself into drill cab, felt pain in left shoulder, pain continued down into left side. Upon arising Fri AM noticed bulge above naval area with pain in abdominal area & left shoulder.
Employee was attaching a chain to a piece of equipment, preparing to move it. He pinched his thumb between a pulley and the lifting chain.
Getting off of equipment and slipped on ice.
Employee was cutting skirt board rubber and inadvertently cut his left leg.
Climbing ladder to dust collector, when at top of landing reached up 4 feet and felt pull in right elbow.
Employee was loading truck with loader when the boom fell jarring the employee. He was taken into the ER and taken off work. Ongoing.
The employee was cleaning the primary when his right foot slipped under the main rotor.
Employee was prying a chunk of frozen material from the top side of the screen with a 3' pry bar when it slipped and pinched his left ring finger between the pry bar and the screen hopper box. The weather conditions were below freezing with mixed snow showers.
While operating the longwall shear, EE stepped down from a relay bar onto the mine floor and injured his lower back. The step down was approx 10-12"". EE finished shift and returned to work for his next shift, but left early because of back pain. Went to DR on 11/30, X-Ray and Rx, returned to work on 12/3. Started missing work on 12/4 for injury.
Bucket of excavator slowly came down while setting electric motor on platform and hit ee's back.
Employee slipped and fell while standing/walking in a scoop bucket.
Employee was working on a dozer, lifting gear sets and got a hernia on his right side. He did not seek treatment until 12/10. He is scheduled for surgery on 12/21/07.
Cleaning under belt with Lee track, sat back in equipment too fast and bumped his elbow.
Scraping build-up off collection hopper of 1"" minus conveyor. Clod of dirt fell and hit above right knee.
EE was helping other ee an old dryer screen toward the hoist well so they could take it out of the plant, in the process of rolling the old dryer screen he tripped and fell causing laceration to his left hand and leg.
Injured was going in mines on D.C. Mantrip. He was sitting on wire side. Trolley pole caught on something on mine roof, pulling pole out of base. Pole fell, hitting EE on right shoulder, causing bruise.
While operating #380 haul truck towards the 2850 dump, the truck started to lose power. As the truck started to roll back, employee applied the brakes and attempted to turn into the highwall. At that point the truck rolled onto its side. He received fractures to the right leg, wrist and a few ribs.
The employee thought that he could make it on one step going down. He could not reach the catwalk and he put a lot of pressure on his hand and knee.
Injury ee was attempting to hang the continuous miner cable by himself, when he felt a sharp pain in his lower back. He was seen by the local emergency room physcian and removed from work.
While pulling wire through a conduit pipe on a 500' run with a p/u truck, the rope we used to pull the electrical wire with was tied in a square knot around a 6"" C-clamp which was attached through one of the loops on the trailer hitch assembly. The clamp twisted off the hitch assembly & hit the employee above the right knee on the side of his leg leaving a very small cut.
Employee was helping change battery on his dozer. His foot slipped and caught his hand on the battery door. It cut and broke his pinky finger on his right hand.
MBC operator backed up from miner that was in #7 heading stopped MBC 8ft inby 6 to 7 brk. Didn't inform anyone that he was going to install cover on front of MBC. Miner operator backed up to intersection, swung miner around to go into 6 to 7 brk. struck operator on left shoulder.
Employee was accessing the splitter box and slipped. He was holding a beam to steady himself and his weight pulled on his left ring finger. Intially it was diagnosed as a strained tendon of the finger. On 5/15/08, minor surgery was performed on the tendon. Surgery was done on his off-rotation and he will return restricted duty on 5/22/08.
Employees were installing new ducts for transfers points, one of the ducts got jammed ee kicked the duct to free it up, thus hitting him in the upper body. this was no damaged done to equipment.
Employee was attempting to make a road around a stock pile and when scraping with the 240 Volvo Loader he hit a rock causing the loader to stop quickly and the employee to injure his back. Employee has suffered a sprain to his back
Employee was running Lo-Trac scoop. He hit his head on fly board, snapping neck back.
The lifting lug was only welded in one spot of the 6' X 12' X 1/4"" plate steel. This caused the plate steel to be too weak and unbalanced, therefore, it fell on EE when the forklift tried to move it. EE should have welded both sides of the plate steel. The steel plate weighed approx 500-700 lbs, causing EE lower back trauma.
Doing pre-shift in #7 entry and slipped and fell hitting right hand finger on hammer or caplight attached to mining belt.
HE WAS WALKING DOWN BELT LINE, WHEN HE SLIPPED ON WET BOTTOM, TWISTED KNEE. **STARTED MISSING WORK ON 01-03-08**
Co-worker approached the injured EE from behind and struck him behind the right knee, causing him to loose his balance and right knee popped. After continuous treatment, the physician gave him restrictions that caused him not to be able to perform his normal job description. This became a lost time incident on 2/20/08.
Injured was putting waterline together. He was pulling with channel locks. They slipped and hit other waterline which had a jagged edge. When locks slipped, hand caught the edge.
Slipped on ice on stairs. Injured his back.
On 11-23-07 employee was dragging rockdust hose and fell backward on his right elbow. He has continued to work until 1-18-2008 when he started missing due to pain in right shoulder.
Employee was trying to open door on IT 28 loader. He jammed his left ring & little fingers in doing so. On December 25, 2007, a specialist diagnosed employee with torn ligaments and he was put off work. Safety Dept. was not notified until Jan. 2 because of the holidays.
Employee stated that he was uncoupling a 3"" victraulic alum. waterline that was hung on a conveyor tail piece and against a coal rib. Not realizing the there was tension on the pipe when he uncoupled it and he was standing on the loaded side and the line sprange back and struck him on the leg. He was removed from the mine and transported to medical services.
Employee was loading miner cable and twisted right knee.
Employee was splitting rock; something went under his safety glasses and scratched his right eye.
Employee stated that he was roofbolting at the face and a piece of roof fell from between the rib and bolter canopy and struck him on the leg and ankle. Person was transported to medical services and was taken off work.
Overstressed back packing and handling blasting powder
EE was walking down 20-m set up face. He stepped in a water hole and twisted his right knee. EE saw a doct that day and was released to return to work. Later a MRI showed a torn meniscus. EE worked from time of accident until 1/3/08. On 1/3/08 he had knee surgery.
Employee was lifting pump impeller and felt pain in abdomen. Employee went home that night and by the next morning there was a lump in the area. Surgical repair was done on 12/5/07. It was unclear as to whether or not the injury was work related. After reviewing surgeon's notes on 12/27/07, it was determined that the injury was work related.
Employee pinched hand between the head of roof bolter and roof bolt plate. He missed one (1) day due to this injury. This occurred over the Thanksgiving/deer season holiday of which we were idle. We returned back to work on November 28, 2007. His day missed was on Friday, November 23, 2007 only.
While switching a valve, strained lower back.
Employee was breaking down medium duty size tires by hand when he felt a strain in his lower back.
Employee had finished his shift and parked his haul truck; when employee reached bottom step which is fastened to the truck by cables it swung in and his foot got caught under fender guard causing employee to fall backwards to the ground. Employee went to doctor who took x-rays and found three broken toes and a sprained ankle. Employee was put out of work until further notice.
EE reported: Operating roof bolter in face area, had installed the ribline bolt, lowered machine's canopy to swing boom to center bolt location. As he was swinging the boom a small piece of draw fell striking his left shoulder, sliding down his back. He has reported a possible strain to low back region.
Employee was moving equipment. He moved the hitch on a Nodwell trailer- the pin that secures the jack was not in place which resulted in the hitch of the trailer falling on employee's right leg. The hitch broke his right leg. He was taken to Nome Hospital and then transported to Anchorage for surgery to place a pin in leg.
Employee was attempting to slide one of the bulkhead doors inside a rail boxcar with the aid of two other employees - when the top of the door stuck, the bottom part swung out, and when they let loose of the door, the bottom of it pinched the employee's foot to the deck of the railcar. Employee was wearing steel toed safety boots at the time of the incident.
Chemical burn to feet, lime solution 8%. Safety gear provided not used ie. rubber boots. Third degree burns in areas of the feet. Antibiotic (cephalexin) and, (Oxycodine) pain killer were prescribed. Has not been released to come to work at this time.
Employee was sitting in a Caterpillar 777F 100T haul truck getting loaded by a Caterpillar 992G loader. With the full bucket of the loader over the box of truck, the loader operator pushed the wrong lever causing the bucket to fall onto box of truck. Truck driver was tossed around in cab causing strained neck muscles and a sore thumb.
While getting out of the truck to buy lunch, employee slipped and fell to the ground landing on his back.
EMPLOYEE WAS CROSSING OVER CONVEYOR BELT WHEN BELT STARTED. TRAVELED APPROX. 25' BEFORE BELT WAS STOPPED. SOFT TISSUE INJURIES TO BACK AND RIGHT HIP.
Employee was operating shuttle car when he came to a 6"" ledge and hit his head on canopy then his neck and back started hurting. Reported to operator on 11-19-07 but never missed any work until 11-26-07 over incident.
Employee was helping to remove a return idler from a conveyor, when the idler fell down and cut his finger.
While watching his cable, employee put his hand between the boom and the frame of the shuttle car. When he let the boom down, his left hand was in a pinch point. He injured his pointer, index and ring fingers. Open wounds and avulsion of the fingers.
Employee was hanging curtain when he blacked out for a second and fell backwards, injuring his back.
The fuel hose was strung all the way out. The hose blew off the reel. Diesel fuel splashed the employee in the face and body. 100psi
Employee was pushing a pallet that had become stuck on the rollers when he felt a pain in his lower back.
Injured was bolting top when a rock fell from the roof striking him on his left shoulder and back. EE continued working until 2/5/08 at which time he started missing work.
Employee's shift began on 11/18/07 at 7:00 p.m. Employee was operating a haul truck and halfway through his shift began to experience discomfort in his back.
Employee strained his back lifting two bags of rock dust.
Using a pick hammer to pound on a fish bolt plate to get it out, a piece of metal off the hammer head flew striking and lodging in his chin area. Cause of this injury was using the wrong tool for the job and safety awareness. Compliance with rules/regs was not a factor, protective equipment not a factor, miner knowledge could be a factor, mining equip/systems not a factor.
Employee was placing a bag of product on the conveyor belt when he felt a pain in his right wrist.
Employee stated he was climbing up a step ladder when he fell off the ladder resulting in a right knee sprain. Underwent arthroscopy right knee surgery on 1/16/2008.
Employee tripped over a 7 inch rock while leaving the restroom.
Employee tripped while he was crossing over a cement road barricade, injuring his arm.
2B 5 North - Employee was hanging hi line when back started hurting. Employee stayed on job and came out of mine and claimed back injury. On Dec 19, 2007 and after medical review and investigation, it has been determined this accident is reportable.
Employee was shoveling belt line while on his knees twisting his right leg at the knee.
Employee was operating Haul Truck #529, being loaded by the #10 shovel. On the 3rd dipper, a large rock was loaded into the truck. The boulder landed on the truck canopy, causing the employee to be jarred. After the incident, the employee was released to work. She was re-evaluated at the Clinic on 12-5-07 and given restrictions not allowing her to return to work.
The man was pulling the trailing cable from the cable reel of the roof bolter when he slipped and twisted his knee.
Employee was attempting to get into the wheel loader with muddy boots. When he got onto the step or ladder of the loader, he slipped and hit his right knee on the step and fell to the ground. He did not seek medical attention until 11/20/07. He was treated & released.
Ee stepped off scoop and twisted his knee and foot. X-rays revealed a broken bone in his foot. No surgery required, only a cracked bone, but will be off regular duty for a few weeks.
When heading to the crusher suppression system discharged. There were no signs of fire. Employee retrieved bag for breathing treatment for asthma, 30 minutes later called for aid, not feeling well. He was given oxygen and taken to the ER, and released back to work. Prior to next shift, went back to ER, taken off work until 11/22, currently ongoing.
He was pouring concrete and felt burning sensations in his feet, ankles and legs.
Employee was jarred in cab of CAT 992 when a haul truck inadvertently backed into him.
Employee was stacking pieces of stone on a wooden pallet. When he turned around the last stone he stacked rolled off the pallet and landed on his foot.
While he was tightening a bolt, he added a tube to the extension handle to add strength. The handle slipped and hit him on the small finger of the right hand.
Employee was descending concrete at Admin building. Near bottom of stairs employee stepped on debris twisting her ankle and fell catching herself on handrail.
EE was climbing onto a Komatsu 475 trackhoe to start it when he hit his knee on the track. He reported it to us and he said it wasn't bad enough to go home. He never missed a day till 1/14/08. He has went to doctors and they done a minor surgery on it. EE will be off for couple weeks.
He was opening a lid and turned it the wrong way and pulled lower back.
EE received two fractures in left arm at the shop after the 100 ton haul truck plate popped out and struck him while he was using an air arc to cut floor plate loose.
Checking the bank, a rock rolled out and hit the employee. He as approx 40' from the bank.
Victim climbed approx. 3 feet up crusher cribbing and fell to the concrete head first Witness to the accident says he went limp before he fell. He had a small cut at the hair line and a bloody nose. Eyes were not dilated. He said his only pain was a headache.
EE pulled muscle in upper back just below left shoulder blade while stacking bags of product on a pallet. One day of lost time resulted.
EE WAS PUTTING BLOCKING UNDER THE WHEEL OF A ROOF BOLTER THAT WAS STUCK. HE GOT HIS FINGERS FOULED UNDER A HALF HEADER AND WAS RUN OVER BY THE ROOF BOLTER TIRE, RESULTING IN TWO(2) FINGERS BEING BROKEN.
Employee was carrying empty oil cans back to the lube center when he slipped in the mud and fell on his knee. Lost time injury December 13, 2007.
The employee was descending the boarding ladder of a Hitachi Backhoe (unit 934) when he lost his footing and fell from the second or third rung from the ground. The employee struck his left arm and head on the ground, fracturing his left wrist and causing a small cut above his left eye from his prescription safety glasses.
Employee was tramming the MBC forward. He ran over a 4"" x 4"" post 36"" long causing the operator compartment to move suddenly upward. Injured hit his hard hat against the roof compressing his neck. He was hospitalized for observation and found to have a chipped bone in his neck.
Driver was dumping coal truck. The hoist assembly at the top busted metal out and the trailer bed fell back down onto the frame of the trailer.
Gradual oncoming knee problem from crawling on knee surgery 11-14-2007.
While waiting for the man hoist at the end of the shift, employee was struck by a roof bolt that was swung by another employee.
Employee started to climb down from a grader after moving the grader in parking lot. Pulled or torn ligament or muscle above right knee cap.
He was lifting the miner cable out of a cable hanger and felt pain in his abdomen. He continued to work at his regular duties until 11/20/07, when he was removed from work for hernia repair sugery.
Walking on the lower floor of the wash plant. Someone had removed floor grate to let wash down water exit. The grating was not replaced, was not marked at all. Later the employee was walking down that same grating and stepped into the open grate that was covered with water.
Employee was removing jib from crane. As jib swung around employee caught finger between jib and crane boom.
Employee was sliding into pant leg chute/he grabbed the rail to stop his slide & strained his left shoulder and neck.
Employee was riding along # 2 Conveyor Belt when he struck his head on a belt hanger, causing pain to his neck, left arm. The employee worked until 12-19-2007, when the doctor removed him from work.
Injured was hit by piece of rock which fell from top striking injured on head and then on right knee.
The man was kneeling beside the roof bolter and when he turned to stand up he felt a pain in his knee.
Victim was assisting workers who were re-railing an off the track 9 ton motor. He was placing wood blocking material under the locomotive when the jack that was set under it kicked out and the motor dropped. His right forearm became caught under the motor resulting in a severe contusion.
EE possibly strained stomach after trying to sit up from a flat position on his back while in the pit underneath a PC 400 Rockbreaker to repair a loose wire. EE did not go to the doctor until November 21, 2007.
Splicing electrical cable, cutting neoprese with knife. Laceration top, left hand developed skin abscess. Did not report or seek medical attention until 11/17/07.
EE was operation a shuttle car in an area where the continuous miner cable was supported from the mine roof. He turned a curve the mine cable was jerked loose from the mine roof, falling onto the operation deck of the shuttle car. The cable struck him on the neck and shoulders. He was transported to a local E.R. where he was told not to return to work.
Employee was opening hood on bulk truck when he felt ""pain in his left side"". Worked throughout the day, but went to hospital the next day as the pain was getting worse.
Operator was reeling up extra cable on the roof bolter. The cable slack came up striking the employee in the jaw resulting in a fracture of the jaw. Employee started missing work on 11/13/07.
Employee was replacing o-ring between wheen and tire ring lock when the slate bar he was using to pry the ring away from the wheel slipped resulting in the employee's finger being pinched between the wheel and lock ring.
Fell as he was dismounting a front loader, lacerating left hand.
EE was moving rock when he slipped on a water puddle and twisted his left ankle. He was sent to the doctor and instructed to stay off of his ankle until the swelling decreases.
Injury ee was walking around the front of the section feeder when he stepped on some loose coal causing him to loose his balance and fall on his right foot, he was seen by his family dr and removed from work 11-13-07.

He stayed on his feet, did not say anything, just motioned to area hit. No blood, no laceration. After safety meeting he told production manager, area still painful. Drove himself to doctor.

Employee was stringing chains over an I beam. One chain was pulled up and struck the employee on the right side of face.

Traveling on a rubber tire mantrip at the end of the shift. The mantrip cut close to the rib to let the 1st shift go by. The rib caught the toe of his boot twisting his right knee, right ankle and right foot.
Retrieving core tube and the rod holder did not fully close on pipe. The rod slipped 4' and ee's hand was behind the moving rod. Broke bone on left hand.
Employee was in the process of cutting a piece of rebar, when the rebar snapped back and hit the employee in the chin and neck, causing deep lacerations.
Our employee was asked to assist Pacific Aggregate Employees to replace a broken shaft on a conveyor. The belt had not been secured correctly to prevent it from rolling backwards. As it rolled it raised the counterweight and pinched our employee's foot between the counterweight and an angle iron support member.
Employee was standing on dock directing inbound vessel for off loading stone when the vessel's mooring line broke and swung down the dock striking the employee. Employee was knocked off his feet and sustained soft tissue contusions.
Employee was installing a roof bolt. He bent the bolt and inserted it into the drill hole. When he pushed on the bolt to straighten it up, he felt a sharp pain in his left side and shoulder.
Employee stated that he and another employee were moving the cont. miner cable out of the center of the entry so the miner could get by. After he was done he felt a sharp pain in his lower left rib cage. He went to the doctor the next day and had a strain to the lower left rib lining. He was taken off work for a few days.
EE was changing a crawler on a Jow 14-15 miner. He was setting a jack when it slipped and mashed his thumb.
Employee was completing his walk-around inspection of production truck and stepped on a jagged rock. He twisted knee and didn't fall to the ground.
Hit bolt with hammer & bolt broke hitting him in left eye. He was wearing safety glasses.
EE was operating a shuttle car and got the cab hung on curtain. He reached with his body through the cab and hit the boom lift with his knee and pinned himself against the canopy. Lost time injury.
He was pulling on pipe wrench while installing bars on the Hailsmith. He smashed his right index finger between the platform and wrench.
The employee was pulling belt and tripped. He fell on a belt rail bruising his left rib and right knee.
#3 section #2 entry spad # 1505. Employee was loading roof bolt supplies out of scoop on roof bolter. He reached to get a bundle of plates. He twisted his back.
EE was building a new dumpster in the shop. While turning the dumpster over something popped in the bend of his elbow. During a follow-up examination on 11/16/07 it was determined that a tendon was detached, surgery is required to repair the damaged tendon.
When driving mantrip out hit small rock with wheel making steering wheel hit his right hand. He went to hospital. After he went home he called back and said it was just bruised.
Employee states he felt pain in his low back while removing rock from a feeder conveyor chute in the Washplant. He continued to work and later requested medical care. He is scheduled for surgery 01/09/2008.
The employee was riding in a mantrip as they exited the mine at shift end. At an intersection, the mantrip hit a rib with the right front end causing employees head to snap back and forth hurting his neck. He suffered a Whiplash that was too sore to return to work.
Employee was operating a continuous miner in #7 entry at spad #681 when a rock measuring 4 x 4 x 5 inches thick struck him in the head. The rock slid down right arm onto the back side of legs knocking him unconscious.
Employee was hooking a chain around a piece of pipe to lift a roll of belt. He threw chain up and over pipe. Chain came back striking employee. Went to doctor 11-9-07. Started losing time on 11-10-07.
Employee was working on replacing a twenty amp circuit breaker. Employee de-energized panel by turning 100 amp main circuit breaker off. Leads on primary side of main circuit breaker shorted and caused flash.
The EE was locking-out power on the MMU-001 feeder. He was bent over looking down. A piece of rock fell from the roof, striking the back of the EE's neck. The EE sustained a concussion.
EMPLOYEE STATES HE WAS HAULING SUPPLIES WITH FORKTRUCK. HE STRUCK A BUMP IN ROAD CAUSING HIM TO STRIKE HIS SHOULDER AND ARM AGAINST CAB FRAME. SINCE THE DATE OF INJURY, NO RECORDABLE MEDICAL TREATMENT HAS BEEN PROVIDED; HOWEVER, EMPLOYEE INCURRED LOST TIME DUE TO THE INJURY ON 03/26/2008.
Employee was putting hydraulic oil in dozer tank. Oiler was handing hose to employee and valve was accidently hit which opened ball valve. Oil sprayed out under high pressure cutting glove and hand.
Employee was filling scoop batteries with water and mashed his finger when trying to replace battery cover.
Employee was running a loader, employee stepped off on to a small rock. He got back on the loader for 30 minutes. Later he complained of knee pain. He was taken to the doctor. Doctor found no damage but there was arthritis and bone spurs. Employee has a history of previous work and non-work related knee injuries. Workers compensation is investigating this incident.
Employee was hanging miner cable and fell off scoop. He twisted his right ankle.
EE reported: In ram car charge station, operating ram car performing task of changing battery. Prior to articulating frame of car, he engaged the battery release hook, this allowed battery to suddenly roll off the car frame. The sudden release of the battery resulted in the car frame quickly raising. This jarred his low back resulting in internal pain to lumbar region
#5 Entry, #3 Section. Bolting a rib and the rib rolled out on left foot.
Employee was riding on bus going too fast, brakes locked and bus hit right rib. **EMPLOYEE DID NOT START LOSING TIME UNTIL 11/11/2007 DUE TO INCREASED PAIN**
Employee was cleaning #7 entry with scoop in low coal seam. While moving forward with scoop, he struck coal rib with bucket causing scoop to buckle somewhat in the center section causing him to strike his head on the mine roof resulting in employee complaining with pain in back of neck.
EE got off of truck to start drill, took a couple of steps and stepped akwardly on a rock and twisted knee.
Employee was prying with a bar in a push beam. The bar flew out and hit the right side of his head.
Operator was driving rock truck on 11-8-07 for about 2 hours. On 11-9-07 at noon he reported to his supervisor that he hurt his neck on 11-8-07 while driving rock truck.
Employee was standing on the front of a CAT 950 loader working on a clamshell feeder and slipped, falling approximately 6 feet. He fractured his knee and had superficial cuts on his forehead
Employee was carrying a rock to place on a pallet. He slipped, fell, and twisted a knee.

Injured employee was driving a pin in a jack thru the bracket with a hammer when he struck his right thumb.
Employee was standing alongside a bundle of steel roof props. He rested his right hand on the base of one of these props. The other end of the prop slid off the bundle which caused the base plate his hand was on to pinch the tip of his right finger against another base plate.
EE was hanging line curtain and tripped/fell fracturing left fibula.
EE was hand scaling from manlift on GM Topkick powder truck. When he went to reposition, a piece of rock approx the size of a soccer ball/plate came loose falling approx 8' - 10' and clipped the back of his hard hat. EE had a sore head/neck and was taken to the Medical Center to get checked out. He was treated with cold compress.
Employee was carrying rock to the trash bin. He jumped on a metal step and fell through the step. He twisted his right knee as he fell. There was slight swellig and immediate pain. Upon dr. examination his knee was found to be strained. He was directed to stay off of it for a day then return to work.

Re-railing the FCT unit when the come-along came unhooked and struck him in the shin.

Employee stated that while walking through No.4 crosscut left into No.3 entry, he stepped through the fly pads and stepped off a 16"" ledge, twisting his left knee. He stated that he looked up at the mine roof because a piece of rock fell on other side of flies and he did not notice the ledge.
The shearer was parked in the headgate area with the headgate drum at shield # 5 and the tailgate drum was at shield # 12. Employee bits on the tailgate drum, he reached down to get his hammer when, a 1?x 4?x 4? piece of face rock and coal fell from the face and struck him from behind.
EE was splitting a slab of sandstone that had a fracture crack in it, which he did not notice. While splitting the stone, it broke in half, the stone fell and struck him on his right ankle. He sustained a lacertion and contusion. EE will be off work until released by physician.
Replacing dog bone that was broke while getting ready to raise pan line, using shield member, had his arm lying on panline shield to hook up chain. As he was lowering the shield, base dropped, causing shield to fall 8 to 10 inches and pinned members left arm between shield and pan line.
While the hydrascreen conveyor was turned on EE tired to pull out a rock that was caught up in the machine. Machine grabbed EE's hand causing a superfical abrasion on his upper right wrist. The loader operator became aware of what was happening, shut down the machine and help EE out.
While changing right rear tire on Dodge pickup truck, sidewall blew out injuring right wrist of employee.
Employee was working on a scoop when his foot slipped causing him to fall backwards his right leg became lodged between bucket jack and scoop twisting leg it broke small bone in his right leg under knee.
Employee was using a wedge and sledge hammer to put pressure against welds in the clinker cooler in order to free up walls of clinker cooler. He felt a strain in his right side (back and ribs).
While walking up the stairs on T-23, employee was pulling a 1/4"" X 95' grease hose when it got caught on an old screen lying on the ground, causing his right wrist and shoulder to jerk in a backwards motion.
EE felt something go into his eye while he was using a cutting saw. EE rubbed his eye thinking it would just come out. EE later noticed that the irritation did not cease, he went to look at his eye and noticed a small blemish on his eye. EE was taken into the ER where he was prescibed eye drops and referred to an eye doctor. He was taken off work til 11/12/07.
While climbing steps of drill, He slipped and landed on both feet.
Employee was inside the cargo compartment of a river barge, when he slipped and twisted his ankle. He suffered an strain injury to the right ankle.
Employee was operating a Lo-Trac (Light Duty Forklift) on the #1 section. As the employee was tramming the Lo-Trac, his right arm was caught between the mine roof and the frame of the equipment.
While loading hydrated lime, employee turned the system off. However, the lime continued to flow after it was turned off. The control room became very dusty so he went outside the control room where conditions were very windy as he tried to get out of the dust. Employee had lime blow into his face receiving chemical burn to eyes and skin.
Injured was walking on section and a protruding cable bolt end caught him on the right ear causing laceration needing stitches.
Employment stated that while walking around the roof bolting machine a pieces of rock fell from the mine roof striking him on the leg.
while torch cutting a section of 8"" pipe to be installed ee was attempting to reposition the pipe when it fell off the blocking fracturing his left middle finger.
While using a table saw to cut 4"" x 4"" blocks of wood, the wood kicked back. The block hit his left thumb, pushing back. The thumb was broken, requiring surgury to repair. Security(contractor) was assigned the duties of safety and reporting. A safety audit identified this accident as not being filed at the proper time.
EE was working to abate an electrical violation on a garage door located in the shop. He set up a 12' extension ladder to climb up and remove ceiling tiles. As he was descending, the ladder began to slide. EE fell approx 6 ft to the ground. He was transported to the hospital where x-ray revealed a fractured left metatarsal.
Employee was walking across yard and tripped on a rock.
The EE was working on the DC on the bin building. While walking down the stairway he fell. He sustained a bruise to his left heel and twisted left ankle.
Employee was standing on #41 Shield, F-15 Longwall when a piece of rock 14"" x 15"" x 5"" fell from above #42 shield and was thrown from the headgate drum of the shearer striking him on the nose and forehead. Employee sustained a fracture to his nose.
Miner was lifting generator out of control room hurt back while lifting generator.
Transporting materials on forks of bobcat skidster, losing a crib block, stopping to pick it up he stopped behind another material hauler. That operator did not see the skid steer behind him and backed up pinning employee between the two pieces of equipment.
Employee stated he was taking a roll of line curtain to the face, when he stepped in hole and twisted his Right knee. Employee was transported to medical services ER and saw his regular doctor on 11/06/2007. He scheduled off the next three days and started missing on 11/10/2007. Results of MRI was recieved 11/16/2007 from Dr.
Operator backed over a rock at the dump site, turning the bed of the truck on its side. Truck bed was turned upright with no damage.
Employee reported on 11/13/2007 that he went to the doctor on 11/12/2007 and was diagnosed with a broken rib. He claims that he slipped and fell from the crusher on 11/05/2007. The doctor allowed him to return to work under restricted duty.
Employee was operating roof bolting machine from operator's compartment when he backed into the coal rib and pinched the trailing cable between the coal rib and the bumper of the machine. The employee allegedly received an electrical shock. Miner proficiency was a factor in the accident, mining equipment not a factor.

EE was working on the seko foam supression building. He was allegedly installing steel siding and experienced back pain. He was taken off work on 11/27/07 by a doctor. He has has a history of back problems.
Miner awoke Monday 11/5/07 with right knee pain. He reported to work for his normal shift. Miner stated the pain was not work related and decided to go to his doctor. Miner left work at approx. 7:00 AM. The next day, after his Dr. visit, the miner notified Human Resources that the injury may be work related. He was then sent to the appropiate medical care.
He was helping hang miner cable and twisted lower back.
Employee states that over the course of roof bolting and repetitive motion he has experienced pain in his left shoulder area. On 12/13/07 medical verification was received on work-relatedness of this claim.
Injured party was getting off his bulldozer at quitting time, when he slipped as his foot touched the ground, causing him to strain his lower back. It had rained that afternoon and everything was muddy.

Returning to the operators station of the 980 loader after a break. Foot slipped off the first rung of the ladder, causing his left knee to strike the second rung of the ladder.
Employee stated he stepped off the Cat. 910 forklift onto the diesel scoop. Footing slipped. Fell off top of diesel scoop, approximately 3 1/2 feet high. Dislocated right shoulder.
EE standing under heat exchanger working. Right foot on lower slope foundation and left foot on elevated foundation, about 12"" drop. 4"" cold roll steel about 12"" long used for counterwieght lying on upper foundation. EE accidently bumped counterwieght with hammer causing it to roll off landing on top of right foot. Resulting in fracture.
Miner had loaded shuttle car and miner operator was on knees at end of miner boom along rib line. When shuttle car was pulling out, the s.c. operator swerved s.c. to right and rear of s.c. pulled toward miner operator, catching his feet with its bumper/frame, injuring his left foot.
Employee was lifting an acetylene tank in the Enlow Bottom Shop and felt a pain in his lower back.
Employee was a passenger in the rear seat of a van. Driving along, the van hit a pot hole in the road causing the employee to strike her head on the roof of the van.
Employee fell from a ladder from the side of a 785 hauler hitting the service truck with his head & chest. This required staples. Note: This accident reported on 4406999 is not correct.
Employee walking on uneven ground and twisted ankle.
The employee was pressure washing the caterpillar 325CL backhoe. The employee was standing on a step ladder to reach the boom area. The employee was approximately three feet off the ground. The employee leaned over the side of the step ladder, it then tipped over, causing the employee to fall to the ground.
While walking to get to shuttle car, ee slipped on pavement twisting his right knee. Person finished his shift, went to doctor after work. First day off work for injury 11/6/07.
While backing the mine out of number 6 entry, ee whom was facing in the opposite direction from which he was walking, stepped on a rock and twisted his right knee. He was removed from work on 11-21-2007 by physician orders.
Employee and a co-worker were using a chain hoist to pull belt, when the hoist hook pulled loose and the chain and hook recoiled, striking injured on the right side of upper torso.
The belt drive take-up was fouled on a shieve. The injured was attempting to free the rope by striking a hammer held against the rope with another hammer. A metal fragment hit his right eye above his cheek and below his lower eyelash.
Employee was prying with pry bar on head shaft of crusher feeder, the bar slipped causing the employee to fall backwards. Employee struck middle of his back against a cross member of crusher.
Employee was at the north stacker taking a cable off of a dozer. He bent down and felt his left knee pop.
Injured was bolting with jackleg, when he went to move it he felt a pull in his groin. The injury was reported, but he said he was fine and kept working. On 12/17/2007 he saw his family physician and surgery was performed.
Cleaning tail gate of dump truck, his right thumb caught in tail gate. Reportable fracture, right thumb; lost time.
While replacing spring pedestals in the #1 refuse dewatering screen, ee's feet slipped causing a strain to his abdomen. Marfork Safety was notified that he did not return until 11/19/07 on 11/12/07.
Employee was drilling roof bolt hole-rash & rock fell from roof striking him on back of left leg. (Rock & rash slid of the canopy)
Employee, shovelling spilled material off belt line, felt groin pull. No treatment. Wrote up groin pull again on 12/26/07. Still no treatment. On 1/10/08 employee decided to go to doctor. LDW was 1/11/08. LTA as of 1/14/08.
Employee was using a bar to unplug a feeder chute. The bar kicked sideways and pinched his fingers between bar and chute. Employee was taken to a medical facility.
Employee was attempting to unclog powder from the hose that was connected to the discharge auger. The hose came off the discharge tube and while trying to avoid the falling tube, he twisted his right knee.
Injured was removing a staple lock fitting staple. Injured was using a hammer and chisel to knock out the staple. A piece of metal from the hammer came off. The metal piece went under injured's safety glasses and struck him on his right cheekbone and right eye.
Injured EE was in the process of getting ready to install permanent roof supports in #6 heading. While in the process of placing the plate over the resin rod and tossing them toward the front of the roof bolter, he felt a sharp pain in his lower back. Injured EE was seen by the local ER physician and removed from work.
Injured employee and another employee where in the process of hanging the continuous miner cable and water line when injured employee felt a sharp pain in his lower back. He was seen by a local ER physician and removed from work.
Employee was rock dusting using 50# bags of dust. He stepped on uneven bottom twisting left knee. He continued working until 1/4/08, his first day missed.
Employee was sitting against the coal rib watching the roof bolter operator tram the machine backward when the machine jerked toward him grazing his left knee. A sharp piece of steel on the machine made a laceration on the top of his knee that required numerous stitches.
Scaling roof back, rock came down, stepped to side and lost balance. Fell and injured left ring finger.
Performing maintenance on Getman - was getting down off of Getman - pant leg caught on filter clip causing knee injury (right knee). Medial and lateral meniscus tear.
Employee was greasing the Wasco hilift. He was using a lever action grease gun. A bucket pin was hard to take grease. He was pulling the lever towards him, with the body of the grease gun resting on his chest. Another employee assisted him pulling the lever. The force against his chest cracked a rib on his upper right chest near his sternum.
The scoop operator was pulling a belt skid, full of belt hardware, for a belt move. A piece of belt hardware fell out of the skid. The EE got out of the scoop; when he moved the spreader bar from between the rib and skid, the skid moved over his left foot. This resulted in a fracture on top of his foot.
Employee was loading a bag of media used in the grinding process and slipped on ceramic beads on the floor. Employee put his left arm down to break his fall and fractured his wrist.
Driving 4 wheeler, hit a rock. The steering wheel whipped around hitting hand.
Conveyor had been running for approx. 10 minutes to warm up. Due to frost, the belt was slipping. He gained access to head pulley by standing on the handrail and placed his left hand over the existing guard. The pulley entrapped his hand and it was crushed.
The employee was taking the belly plates off of the 963C Cat trackloader when one of the plates fell on his right index finger and broke the finger.
Employee was bolting top, when a piece of coal with rock fell from the rib/roof and hit him in the side/back, pushing him into the roof bolter controls.

Employee was operating 100 ton rock truck. When dumping, the truck blew a hydraulic line. The fluid sprayed on the engine causing a fire. When he exited the cab he burned his arms and head. He also hit his head on the way out of the cab causing a cut that required stitches.
Bending roof bolt and slipped. Roof bolt hit him in chest on right side.
Muddy conditions. Stepping over a coupling on a flat car, EE tripped on the coupling. His right ankle hit the track.
Employee stated he was operating a shuttle car, struck bump in haul road, felt pain in lower back. Started missing work on 11-5-07.

While walking in Number 3 entry, employee claims to have slipped and fallen, twisting his right knee. He was removed from work by Doctor on 11-20-07.
EE was standing in last open X-cut in unit. A piece of rock fell from between the bolts, striking him on back of head and neck.

Stepped out of Unit #89120 (quad) onto a rock, twisted left ankle & fell onto his back.
Employee came down stairs, when he reached ground level, he turned to go to maintenance truck and stepped into a crack in the floor pad with the outside of his foot and twisted his right ankle.
Employee's glove got caught on the roof bolter drill steel, twisting his right hand. His last day worked was 4/15/08 and started missing work on 4/16/08 for hand surgery.
Worker was changing a return roll on conveyor in mill when he strained a muscle in his shoulder. Medical treatment was limited to diagnostic only. Injury did not become reportable until worker's first lost day, Dec 4, 2007
EE had just gotten back into the cab of his loader after talking to a truck operator. He placed his hand on the door jamb and a wind gust came up slamming the door on his hand. Three fingers were cut and two were broken.
Employee was replacing hydraulic line on Terex loader. When taking bolts loose, the guard that covered the line fell on his foot. Employee reported accident to manager, refused to go to doctor, has missed two days of work. On 11/30/07, I made a doctor's appointment for him. He said he would go, but didn't show up for the appointment.
Blaster did not have good visual of area below: parked equipment blocked access into the blast area. After first blast, loader went into area to get a bucket of dirt. Second blast went off. There was damage to cab of loader. Injuries to ribs and head of operator.
EE was injured on 11/27/07, when a piece of stone broke away from a large slab of stone he was placing on a pallet and struck his elbow. EE declined medical attention at that time. On 12/12/07, EE requested medical treatment for the injury, which occurred on 11/27/07.
While employee was lowering a lift cylinder onto a loader, employee tried to line up the cylinder. His finger got caught/smashed between the cylinder and the loader. No property damage, could lose a portion on his right ring finger.
While changing screens, employee reached under screen to retrieve a bolt while screen was being lowered. Screen edge hit employee on shoulder& back of neck causing bruise / contusion.
Employee was hoeing out a chute on the crusher and hurt his back.
EE was installing a water bottle on the water cooler when he felt a cramp in his right bicep. He later noticed that his bicep had a bulge in it. Surgery was performed and it was determined that EE had bone spurs that caused the tendon in the bicep to become frayed. Bone spurs are the result of long term work stresses. The EE did not receive medical care until 12/12/2007.
Injured was removing sign from access platform when he felt burning pain in his arm. Did not become reportable until he started to miss work due to surgery on 12-06-07.
EE stained his back while taking a screen out at product load out. Restricted to light duty on 12/6/07 (2 weeks).
We had completed our shift and gotten into a 4 man ride to exit the mine. The rest of the crew loaded onto the 12 man trip to exit. The operator of the 12 man ride accidently sent the mantrip into the rear end of our ride. His foot slipped of the brake and onto the accelerator.
On 11/20/2007, employee was checking scaling ground in his work area and strained his right shoulder. He continued to work as normal until 1/2/2008 when it was surgically repaired.
Employee was walking down stairs and on the last step to floor twisted right ankle.

Employee was jackhammering when a piece of hot material fell from above and hit him on the neck and head. He suffered burns to his left hand and neck area as well as a broken right thumb.
Employee was roof bolting, when the rib rolled out, pushing his right leg into the roof bolter.
The accident/illness was reported on 01/02/08. The accident/illness actually occurred on 11/20/07. Surgery to remove a blood clot was done on 12/14/07.
Accident occurred on 11/20/07 employee did not seek medical attention until 11/30/2007. Surgery was 01/28/08. Employee was decending stairs on a belt conveyor line when he slipped while grabbing handrail felt strain in his lower back,Rt. side and groin area. It was determined employee had an umbilical hernia. Employee remained at work at his job until his surgery date.
Employee was rock bolting the ceiling. The ceiling let loose and rock fell from the ceiling, striking the employee on the shoulder and in the chest.
Employee went to company clinic on 1/3/08 and was returned to full duty. On 3/4/08 employee returned to the clinic and was placed on restricted duty. The employee was inspecting the plant before start-up and slipped on stairs resulting in a left knee sprain.
He was pounding rocks out of the jaw with a sledge hammer. When he hit some loose rocks the hammer pulled him forward and strained his back.
Employee was cutting a splice on a cable with a utility knife and cut his left index finger.
Employee was changing wear caps on the shovel. When he attempted to strike the old cap to loosen it, a metal chip struck him in the face embedding in his cheek. He was wearing proper PPE.

Employee states he was walking along the rib returning to the CM when the rib sloughed off striking him on the right side of his back and knocking him down. His left leg was pinned under the material from the rib. Employee is scheduled for knee surgery 11/27/07.
She was getting off a jeep and tripped, falling to her knees. She strained her left knee.

Employee was tightening bolts on the side frame of a shovel. He was using a hydraulic torque machine when a stop block from the shovel broke off landing on the employee's left foot.
Roof rock fell out and hit drill steel. This caused the drill steel to bend. The bent drill steel hit employee's left hand on the back side. Returned to work 11-28-07.
Chains were being removed from a fender to unload a truss from a truck. The employee was standing on the truck bed of the trailer when the load jumped up, knocking him off the trailer, causing him to sprain his ankle.
EE was installing a collar pipe in lifter holes, when he bent over to install collar pipe to prevent filling lifter holes, he was struck upon the back of the head/neck/shoulders by a piece of loose rock. Drill Jumbo was being utilized at the time as the method of drilling. Resulted in a laceration to back of head, abrasions and contusions to neck and shoulders.
I was lifting a 60 lb. roller for a converger to get it out of way. I must of been twisting as lifting. Back popped and I fell to the ground. Could not get up for a couple minutes, but when I did get up, the pain was bad and I could not walk very well.
Employee was working on barge dock loading empty barges. Employee was bending over to pick up an accudraft box which weighs 5-10 lbs. He stated when he stood back up he felt a sharp pain in his back. Took employee to get examined on 11/16/07 doctor did not prescribe any medication but did place EE on light duty for a week. EE's personal doctor excused him from work on 11/21/07.
While removing a bearing the puller failed causing part of tool to strike hand.
The injured was drilling a roof bolt hole when the drill steel bowed out and struck his left hand. This resulted in a fracture to his hand.
EE was working at the pole yard, changing out the cable reels. As the EE wa spulling the center pin from the reel jack, he felt a strain in his right upper arm. EE was taken into the ER, where he was taken off work and referred to a specialist. On Going.
Employee was digging holes for new fence, put in (hole digger) and twisted his lower back.
The individual was walking down the mine entry when he tripped on a rock which caused him to fall. He struck his left knee on the mine floor.
Installing roof bolt, after t bar was set, a piece of rock fell from between t bar and last row of bolts. Rock size was 48"" x 16"" x 6"". He was hit from front and knocked backward. He was struck on shoulder, chest and right leg. He returned back to work the next day. 1/21/08 he called and stated his dr advised him not to work.

Employee was loading stone from the mill. When he opened the truck door to open the bin gate the truck rolled back and caught his left arm between door and mill frame. Employee thought truck was in gear but it was not.

Employee was using a come-along; when the hook on the structure broke, the come-along became loose striking his left wrist. Lost time reportable injury 1/23/08.
