The fall was located on Grassy Creek Mains at crosscut #146, SS# 23226 in #4 entry. The fall measured 30'L x 30'W x 12'H. No injuries and no equipment involved. Ventilation not impaired. Fall will not be cleaned up.

A roof fall measuring 6' x 20' x 20' was discovered on 1/31 during a weekly examination of old airways. The fall was located 1 crosscut inby spad #152. The fall area has been dangered off and the fall will not be cleaned up.
The continuous miner was stuck and drawrock fell on the miner before it could be removed from the pillar cut.
A roof fall occured in the last open crosscut on MMU 006. The fall was in No. 3 entry at spad 6+30. It was in an intersection and measured approximately 20x24x8' high.
Roof fall discovered on 1/31/08 outby #4 section to #4 entry near spad 1544. Fall was 55' L x 18-20' W x 5-7' H.

The mine encountered an unplanned roof fall on the 3rd Main Northwest, Entry #2, from 51+80 to 53+10. The fall measured 130' X 19' X 8' thick. The fall did not affect ventilation, or the passage of men, no equipment or injuries were encountered. The area was developed on 12/2006, supported with 8' point anchor bolts. The fall will not be cleaned up.
First crosscut #3 intery top was broke up so I cut bolts out to let rock fell.
A roof fall was discovered during mine rescue explorations on 1/31/08 on the Grassy Mains Bleeder entries at Brk #80, #6 entry-SS #14655. The fall was in an intersection and measured 30'L X 30'W X 9'H. The actual date of the roof fall cannot be determined due to the mine being closed, so the date of the accident is the date of discovery.
A roof fall occurred in the 18 North Panel in the #6 entry inby crosscut #32 and extending 20 feet outby crosscut #36. The fall measured approximately 160'x18'x9'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.
The mine encountered an unplanned roof fall on the Main South, Entry #1 from 7+20 to 9+00. The fall measured 180' X 19' X 6' thick. The fall did not affect ventilation, or the passage of men, no equipment or injuries were encountered. The area was developed on 6/2007, supported with 6' fully grouted bolts. The fall will not be cleaned up.
A roof fall was discovered during Mine Ruscue exploration (exploration due to incident at the mine which had it idled for several months). The fall was located on the 3 East Mains Section in the intersection of the #3 entry at survey staion 24992. The fall measured approx 25' long X 20' wide X 5' high. The fall fell across the ATRS pf, a Fletcher Dual Head Roof Bolter
A roof fall was found in the outby area of the South Mains, break # 72 entry 5 was approx. 50 ft. long by 20 ft. wide by 7 ft thick and break # 65 #4 was approx. 20 ft. by 20 ft. by 7 ft. thick. Additional support has been added to the area. The information was called out to the Supt. at 11:51 am.
A roof fall was discovered during mine rescue exploration on 1/30/08 on the 12 RT. section. The fall measured approx 50'L X 20'W X 9'H in the intersection of #2 entry at SS# 25192. The fall caught approx 10' of the continuous miner at the cutter head of the machine. This fall will be cleaned up.
Roof Fall just outby spad No. 448 in No. 4 entry. Fall is 20'X20' approximately 4.5' to 5' high. Located in a neutral entry approximately 50 cross-cuts outby the face in a previous fall area.
2nd Northwest Mains, #4 entry between spad #341-345, 9000' outby section, unintentional roof fall occurred. 30' long x 18' to 19' wide x 6' high, wet weathered soap stone, bolted with 6' resin bolts fully grouted.
We have not had an accident/injury/illness. At the end of our sand filter system, we have a channel pipe directing the water to the silt pond. The pipe joint broke apart causing a wash on the discharge end going to the silt pond or impoundment area which has an emergency spillway.
Hoist bearing was vibrating. Investigator checked bearing and noticed that the south pinion bearing was bad and needed replacing so the hoist was ordered down until bearing was replaced.
Hoist was down due to major fault would not re-set.
HYDRAULIC SLEEVE EXPLOIT CAUSING A SMALL EXPLOSION
2 mechanics were in the process of repairing floculent scres in GR3 process plant when they ignited guar gum dust. No injuries were sustained by either employee.
A fall of the roof occurred at intersection, in # 5 entry at station 18+90 of the Southeast Mains. The fall was approximately 20'x20'x8'. No equipment, ventilation or personnel were affected. The area was developed in April 2004 and had been supported with 6 foot fully grouted roof bolts.
An unintentional roof fall was discovered at 10:30am on 1-29-08. the location of fall is the intersection at spad # 2523 of 3 east & south the #4 Rt Panel. Fall is approx 25 x 25 x?, is timbered and dangered off.
A roof fall occurred in the 7 East panel at crosscut #17 in the intersection of #8 entry. The fall measured approximately 20'x 20'x 6'-9'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.
The bolterman was spinning the resin in a bolt while holding the bolt and plate tight enough to the roof to cause sparks to fall. The sparks ignited a methane seep in the floor. The bolterman put it out immediately with the water hose on the roof bolter.
Unintentional roof fall - old belt heading. All entries in area common. All stoppings in area intact.
Chemical foam was sprayed on cavity and heated up during expansion. The heat created a small fire.
A fall of the mine roof occured in a room off of section #2. The roof bolter machine was parked in this room. There were no injuries and no airways were blocked. Fall measured 15'L x 20' W x 4' H.
Fall was found by mine foreman and MSHA inspector while traveling airways in worked out area.
Employees became stuck on the Livingston Portal Elevator shortly after leaving the Surface landing en route to the mine level. This occurred at 12:05AM. The employees were able to get back to the surface at 12:45AM and everyone got out safely. The elevator contractor made the necessary repairs and the elevator was put back into service at 8:00AM on 1/28/08.
At 7:10pm a plant-wide power outage occurred. The #2 coal mill shutdown loaded with coal. The operator then bumped the coal mill to empty it and an explosion occurred. The booster fan sustained damages and the explosion vents opened.
A roof fall occurred in the 2 Main North, crosscut #24 in the intersection of #8 entry. The fall measured approximately 19'x 19'x 6'. The area was supported with 6' tension rebar bolts and 6""x6"" square timbers. An undetected slip was determined to be the cause of the fall.
A roof fall was discovered in the Main East, crosscut 25, #7 entry. The fall measures approximately 22 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts.
An unplanned roof fall was discovered at 9:45 a.m. on a routine airway exam in the #1 entry of the return entry at Spad Station # 1695. This is app.5,300 ft. from any section. Size of fall is 90ft. in length X 19 ft. in width X 6-8ft high. It was bolted with 6' tourque tension bolts. This will not be rehabilitated but the brattice line will be re-routed.
A roof fall was discovered on this date. The fall is located on 6-north, break 4, between then #2 and #3 entries. The fall was approx. 15' long by 20' wide, and 5' in thickness. the area was previously supported with 5' hy-tech bolts and 8' superbolts. The area will be dangered and cribbed off. Ventilation was not affected.
7 Mains 2, 2 entry, between 33 & 34 block, MMU 001 17 ft wide x 20 ft long x 6 ft high. Support: 6 ft 3/4 inch resin bolts, cable truss bolts, pans, brow straps. Fall dangered off, additional roof support installed around fall.
A roof fall was found at 4:12PM on 1/27/08 at survey atation 60 + 90 entry 5 5RMS. Fall was 20' long x 50' wide x 10' high. Area was mined October 2004 and had been supported with 8' full resin bolts. No injury or property damage resulted and the fall will not be cleaned up.
The Cedar Creek Elevator was out of service from approximately 11:00 am until 3:00 pm due to a power outage at the substation.
A roof fall was encounter during the onshift examination of the slope and # 1 belt location the bottom of the Justice belt slope. This area is located at spad #2057. The fall measures 20 ft. long, 20' wide and 6 ft high. This fall will not be cleaned at this time and was timbered off. 6ft tension bolts were used in mining of this area
North 6, 22 block, 5 entry, MMU 003. 8 ft wide x 10 ft long x 4 ft high. Support: 6 ft 3/4 inch resin bolts, cable truss bolts, screen, pans, brow straps. Fall dangered off, additional roof support installed around fall.
EE was using the Step I Agglomerator shop elevator when it became inoperable resulting in the employee being trapped in the elevator for approximately 3 hours until repair service made the elevator operable again. Majority of entrapment time due to lengthy travel time for service responder (2+ hours).
Roof Fall in Fault, 3 South, #6entry, x-cut 24, fall was 22'w x 25L x 8'-10'hi, the area was bolted with 8' tension rebar and had supplemental support, area had already been identified.
While doing the normal pre-op walkaround, the dayshift hoist operator noticed smoke emitting from the brake coil on the Production hoist. The hoist was stopped and mechanic called. After initial investigation and not knowing repair time, a mine evac was started. Mine evacuated; no injuries/problems. Hoist back in service at approximately 6:35am.
The Tyro Creek Elevator was out of service from approximately 6:00 am until 7:00 am due to wiring connection in conduit box.
A roof fall has occurred on the 1st Northwest Main at SS. 8+40 entry #3. The fall measured 50' X 20' X 20' thick. The fall did not affect ventilation, the secondary escapeway (travel road) was blocked. No personal injuries or equipment damage. The area was developed on 7/2002, point anchor bolts 5' and 12' in length was used to support the area. This fall will be cleaned.
A roof fall occured on the 10A supply (roadway) at x-cut #19. The fall measured 100 ft in length, 19ft in width and 6-7 ft in ht. (No equipment or EE's were affected.) That was the 8th north east mains.
On 1/24/08 at apprx.408am, hoist at Dotiki IV experinced a malfuction with a field current regulator. MSHA was notified at approx. 419am
The Tyro Creek elevator was down from approximately 6:00 pm until 11:00 pm due to the mine door locks.
During an examination of the ventilation system a roof fall was discovered in the North Mains left return number two entry crosscut thirty two. The roof fell several feet above the anchorage. MSHA was notified.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to a large piece of ice falling and damaging toe car door and the bottom level switch. The damage was repaired and the switch changed and the elevator was placed back into serice. The escapeways were re-routed and no individuals were on the elevator at the time of the outage.
An unintentional fall occured above bolt anchorage. The fall was 20' X 20' X 11'. Fall was in the #5 main west entry.
A roof fall occurred in the #1 entry of the 3lem (mmu 002) section. The fall was approx. 40' in length x 18' wide x 6' high. The fall started 30' outby #50 intersection. The top was bolted with 5' resin bolts with wire mesh. The fall partially covered both lube truck #lt235 and fletcher roof bolt #rb002.
Elevator was down due to a bad door motor and ice on the door track.
A roof fall was found at evaluation point ""D"" inby #101 cross-cut in #5 entry of the 33 Mains headings. There were no persons injured nor was there any damage to equipment as a result of this occurrence.
A roof fall occurred in the 7 East Panel, #9 entry, in the pillar line between crosscut #40 and #41. The fall measured approximately 30' x 18'6"" x 5'. The area was bolted with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.
A roof fall has occurred on the 1st Northwest Main at SS. 8+40, entry #4. The fall measured 55' X 18.5' X 18' thick. The fall did not affect ventilation or the passage of men. No personal injuries, conveyor structure damaged. The area was developed on 7/2002, point anchor bolts 5' and 12' in length was used to support the area. This fall will be cleaned up.
Moving screen. When lowering to ground, screen started to slide off forks. Caught one fork and pulled fork lift over onto screen. No damage to fork lift until we pulled it back upright, bent roll cage. Will replace and recertify.
An unintentional roof fall was found on the 6 left section #2 entry at spad #353. The fall will not be cleaned up.
A unplanned roof fall occurred in LW202 tailgate xc 4 from #2 entry to #3 entry. This fell toward the seal. This area was isolated by moving the isolation stopping outby from roof fall.
An unintentional roof fall occured on the East Pillar Section, (MMU 020) in the No. 6 entry, approximately 50' inby Survey Station 11685. The fall extended from the first lift in the left hand block of the No. 6 entry. The fall was approximately 4' high, 25' wide and 30' long.
A roof fall occurred in unit 1 (1st west panel) in the rooms. It is located in return room 2 in the last open xcut. It measures 20 ft wide, 30 ft long & 7 ft thick. This fall did not affect ventilation, travel or cause injury. It will not be cleaned up. No one is required to work or travel in this entry.
A reportable roof fall occurred in 18 Tailgate 31 crosscut, #3 entry. The fall was approximately 40 feet long, 30 feet wide and 6 to 7 foot high.
A roof fall was discovered in the 1 North 1 Mains, 6 crosscut, #5 entry. The fall measures approximately 18 feet long, 18 feet wide, and 7 feet thick. The area is supported with 5 foot fully grouted resin bolts.
Hoist was down due to a bad overload relay and amplifier card.
Hoist was down due to batteries needing to be changed.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT 12:45 PM. MSHA NOTIFIED BY PHONE AT 12:57 PM. FALL WAS IN 2ND WEST MAINS, ENTRY #5, X-CUT #36. FALL MEASURED APPROX, 18'W x 20'L x 5' TO 6'H. AREA WAS DANGERED OFF WITH BREAKER ROWS, AS NO TRAVEL IS REQUIRED IN THIS AREA.
During the shift, the production hoist became inoperable and troubleshooting began. When the problem was not promptly discovered, the mine was evacuated. Investigation revealed that the electric wear switch on the brake accumulator was out of position. It was re-positioned and hoist put back in service at approximately 12:40. No injuries or other problems.
Door switch on elevator failed causing elevator to be down 32 minutes.
There was a roof fall in #3 entry #2 Break Longwall Setup.
Break #64 roof had fallen. 18'wide x 10'high x 50'Length. Roof is shale and sandstone. Falling out above the anchors.
Roof fall 2000 feet from the face. #4 right between #4 and #5 fall was 4' high. 14' wide and 30' long entrance to fall will be dangered off and supported. per roof control plan.
A roof fall occurred in the return of the 3rd West panel. The fall is located in entry 8. It measures 20 ft long, 2 ft wide & 8 ft tall. It is at 11 x-cut. This fall did not affect ventilation, travel, or cause injury. It will not be cleaned up. No one is required to work or travel in this entry.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, #5 SECONDARY ESCAPEWAY ENTRY, BETWEEN X-CUTS 31 & 32. THE FALL MEASURED APPROX. 18'L x 16'W x 18""H. MSHA WAS NOTIFIED BY PHONE AT 9:42 AM.
An unintentional roof fall occurred. 30' long x 20' wide x 9' to 10' high. Was bolted with 6' tension & 8' point anchor bolts at spad #3838, #6 entry 005 mmu, 1-A northwest mains, 2200' outby section. Primary escapeway was blocked, production was stopped. Everyone was brought outby till escapeway was rerouted.
An unintentional roof fall was found approximately 11:15 am on 1/17/08. The fall was located at the intersection of spad 793 on the old # 3 Belt Line. The #3 Belt Line is no longer in service. The fall is dangered off and all approaches have been cribbed. The fall will not be cleaned up. The dimensions are 20x20x5. The fall occurred due to weathering in the mine roof.
Last push out in No 5, pillar fell on top of continuous miner. Caught 1 MRS also when cut went through.
2000' feet from the face at #2 entry the fall was 7' high. 14' wide and 40' long, it will not be cleanup . The fall will be dangered off and supported as per roof control plan.
An unintentional roof fall has occurred in the intersection of the South East Main Return, in # 7 entry, at 30+10 crosscut. The cause of the roof fall was laminated and unconsolidated shale at the anchorage point. The area had been supported with 6 foot fully grouted roof bolts.
A roof fall occurred in the 2 west 1 mains @ 75 crosscut, between #3 and #4 entries. The fall measures approx 18 feet long, 18 feet wide, and 6 feet thick. The area is supported with 5' foot fully grouted resin bolts.

A roof fall was found during a weekly exam. Located in the neutral, in #2 entry Brk. 16 on #10 belt, the size of the fall was approx. 20' x 30' x 5' thick. No equip. was involved and ventilation has not been effected. The area has been cribbed and dangered off.
A roof fall occured in the return of the 2nd South submain entries. The fall measured 20 ft long, 25 foot wide and 8 foot high. It is located in #8 entry at 16 X Cut. This fall did not affect ventilation, travel or cause injury.
Our secondary escape hoist (For Zone 2)was inoperable do to Ice build up in the raise. Pulled EE'S out of the Zone 2 & reversed intake fan to exhaust fan & thawed the ice. Hoist was put back into operation on 1/16/08 at approx. 12:16 Pm. This mine has 3 different escape hoists at this Mine site, please do not say this is a duplicate and reject this 7001 report.
Rock fall occurred on 1 East Mains at break 20 in the #5 entry. Fall measured approx. 20' wide x 40' long x 4' thick.

An unintentional roof fall occurred on No. 7 belt no. 6 entry, break 15 on spad #7065. This was an old fall reported before. Fall measured 10' long, 18' wide, 3' thick. No injuries occurred. Fall occurred 1600' from the face.
A roof fall was discovered in the 2 North 1 Mains, Corsscut #17, #7 Entry. Tha fall measures approx 20' long, 18' wide and 5' thick. The area is supported with 5' fully grouted resin bolts.
Rock fall in return in #1 entry 2nd break on first left off #1 mains at spad #737. Not going to clean up fall.
#3 Service Hoist brake did not release during pre-use inspection. The hoist was shut down for 55 minutes to adjust the brake. It was returned to service at approximately 10:15 AM. Mine crews were evacuated to the surface via the decline upon hoist shut down.
Roof Fall outby working section in return aircourse; # 8 entry 1X inby spad No. 696 & No. 9 entry 1X to the right of spad No. 696. Fall was 4.5 to 5 feet high, 19 feet wide and 29 feet long. Using 4 & 5 foot tension rebar bolts. Approaches to fall supported and dangered off. Did not block passage of air or men. Will not be cleaned up.
An unplanned roof fall above anchorage of origianl 60"" fully grouted tourque tension roof bolts occured at this mine. The fall was app. 80' out-by the #1 entry. It measured 30 ft in length x 25 ft in width and app. 8 ft high. Spad Station # 2792.
A roof fall was discovered in the #3 main 2500' outby the section in a parallel entry. Fall location is #3 main #5 xc #10 entry. No impairment or damage to persons, equipment or systems.
The 002 section had pillared 5 rows without an initial fall. The crew noticed that a fall was impending and moved and timbered the equipment to a safe location. The fall rode through the breaker timbers 1crosscut inby spad 1291 appx. 70' x 20'w x 10' high. MSHA was present at the time of the fall.
Motor starter that feeds hydraulic brake pump motor failed closed. Pump built up pressure, by-pass valve activated, dropped drum brakes (ON) stopping hoist. Men were lowered to landing and evacuated through secondary shaft. Motor starter, motor, pump and valve all changed. Operational at 6:30 p.m. MSHA notified.
Roof fall occurred at the stageloader in the 5 North active Longwall headgate belt entry.
A non-injury accident, the unintentional stoppage of the elevator at Camp Run Portal, entrapping 1 person on-board the elevator occurred. The source of the malfunction was in the circuitry when switching from a generator back to the main power source while testing.
Roof fall 120' long X 19.20' wide X 6.8' high, 10,000' outby #3 section. Fall locatd in #1 Entry near Spad 10371.
Schindler Elevator Company was performing the required 60 day test on the #8 elevator. The elevator safeties failed to completely set during the test. The elevator was taken out of service @11:25 am. The car safeties were cleaned and lubricated. The test was repeated and the car safeties then worked correctly and the car was placed back into service @1:15 pm.
A continuation of a roof fall was discovered in the 2 West 1 Mains, 80 crosscut, from #10 - 12 entries. The fall measures approximately 120 feet long, 18 feet wide and 6 feet thick. The area is supported with 5 foot fully grouted resin bolts.
A roof fall was discovered in the 2 West 1 Mains, 80 Crosscut, #9 Entry. The fall measures approximately 30 feet long, 18 feet wide and 12 feet thick. The area is supported with 5 foot fully grouted resin bolts.
Roof fall above the anchorage on the #10 Headgate Panel at Break #40 Survey spad 3740 in the #4 entry. The fall covered two shields and 1/2 of the mule.
Our pre-shift examiner found a roof fall on the 5 West main entries. The fall was located at crosscut 38, between the travelway and belt line. The fall was about 4 feet high and 35 feet long. The fall area was timbered and cribbed.

A roof fall was discovered in the 2 west 1 mains, 65 crosscut #9 entry. The fall measures approximately 40 feet long, 18 feet wide and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts.
For reason's undetermined at this time a fire started on a D-11 Caterpiller Dozer, No injuries due to this accident.
An unintentional roof fall occurred 1700 ft. outby the section in the #6 (Track) Entry. The approximate size of the roof fall was 15 ft. long x 18 ft. wide x 4.5 ft high.
An unintentional roof fall occurred in the Main South at Entry #4 at +2625 south. The area was supported with 4' conventional bolts. The fall area is approximately 21'W x 20""L x 4'- 5' H. The strata broke just at the anchor point. An evaluation of the area was conducted.
A roof fall occurred in the 2 West 1 Mains, 64 Crosscut, #5 Entry. The fall measures approximately 25' Long, 20' Wide and 9 feet Thick. The area is supported with 5 foot fully grouted resin bolts.
An unintentional roof fall has occurred in the intersection of the 2nd West off of the South West Mains at crosscut # 30, # 2 entry. The cause of the roof fall was laminated and unconsolidated shale at the anchorage point. The area had been supported with 6 foot fully grouted roof bolts.
A fall of roof occurred between the 6th SW mains and the 6th SW Golf cart shack run around. The fall measured 50' x 20'. No equipment, personell or ventilation was affected.
A tail peice with motor and reducer damaged the side of the cage while trying to load it on the cage when it was on the bottom. The motor and reducer were taken off and the tailpiece taken out. The cage wall was further damaged when the tailpiece was unloaded on the surface. Repairs were made and inspection of hoist conducted. Returned to use 1/11/08 3:50 PM
A roof fall was discovered in the 2 west 1 mains, crosscut 67, #9 entry. The fall measures approximately 50 feet long, 18 feet wide and 8' - 10' thick.
A filter on a rockduster (RD04) located at Crosscut 86 on the Main West Travelway of Galatia North Mine ignited at approximately 3:00 P.M. CST. The fire was immediately extinguished with no injury to personnel or disruption in ventilation. This is not a reportable mine fire and is submitted in response to a violation.
Upon mining the #2 heading. The mining machine cut into an auger hole. The auger hole was determined to contain no water or gasses. No auger holes were projected on mine maps. No injrues occured as a result.
Unintentional roof fall discovered 1/9/08 during weekly inspection. Fall located in No. 11 Zone, 18 Block, Xct 8-9 one block inby station 680, No. 9 entry. Fall previously supported with 6' full grout resin bolts. Area mined on or about 10/21/06. Fall measured approximately 17'W X 6.5'H X 15'L.
As water receded in the old Northwest Mains area during pumping efforts a roof fall was identified in the #5 entry. The fall extended 1 break outby spad #4186. The fall was approx. 80'L X 20'W X 4'-5' high. The area contained 42""-48"" resin bolts and was mined approx. 2/94. The area has been dangered off and timbers have been set.
Due to heavy rains the Limestone Quarry crusher building was flooded. All power was shut off and pumping of the water has began. An adjacent creek overflowed its bank causing the flooding of the building.
A roof fall was discovered outby the section in the #6 entry at spad 1281. The fall did not affect persons, ventilation nor equipment. The fall is approximately 20'x20' x 12 feet high. The area has been dangered and timbered off.

Extenuation of Pillar Line between 169 - 168 Block Outby 70' by 7:30 PM extended 70' more, cribbed area and abandoned
A roof fall was discovered in the crosscut between the no. 6 & 7 entries of No. 1 southwest mains adjacent to survey station no. 198. The fall is approx 3000 feet outby the mmu 001-0 and measures approx. 48 feet long x 15 feet wide x 7 feet high. There were no injuries or equipment involved. Ventilaiton was not affected.
An unintentional roof fall was discovered at 2:20 pm. An area approx. 10' x 10' had fallen from around bolts near the mine drift mouth in the primary escapeway.
A roof fall has occurred on the 1st Northwest Main at SS.16+10 entry #2. The fall measured 35' X 18.5' X 18' thick. The fall did not affect ventilation or the passage of men. No personal injuries or equipment damage. The area was developed on 7/2002, point anchor bolts 5' and 12' in length was used to support the area. This fall will not be cleaned up.
A fall of the roof occurred on the 7th 54 belt line, crosscut #189. The fall measured 18' X 15' X 4 1/2'. No personell were affected.
An unplanned roof fall occurred at the #130 mine one break outby spad #2980. The fall measured 20' wide x 28' long x 7' high. No injuries were reported.
On 01/08/08 an unintentional fall of roof occurred on the 3rd North Sub Main, 3rd Main West at cross cut #119 in front of the 3rd North Sub Main #1 seal. The fall was approximately 30' long x 20' wide x 10' thick. The fall will be cleaned up per roof control plan.
Fall in Room off of #1 Entry at spad #388. 20'W x 20'L x 15'H. No injuries, no airways blocked. Area was not on the active section area. Had been cribbed off prior to fall.
Subsequent to the completion of the shaft inspection, the main door of the #1 skip was left in the down position. The door blocked the capability for rock to enter resulting in muck buildup outside the door. The ship then became stuck. The hoist was down for approximately one hour while the muck was removed. There was no damage to the hoist system.
A fall of roof occurred in the 1st Northwest mains at x-cut 107, between # 1 and #2 Entry. The fall was approx 75ft long x 18ft wide x 15ft high.

On 01/07/08 an unintentional fall of roof occured on the 4th South Sub Main at cross cut #32 in entry #5, the belt entry. The fall was approximately 20' long x 17' wide x 15' thick. The fall did not occur on a working section and will not be cleaned up. The fall was due to fractured stack rock.
Roof fall at #19 break on the 8P Section in the #1 belt and track entry 10'hx60'lx18'w. Employess were removed from the affected area.
A fall of ground happend on the main slope of the mine at Black River. This fall of ground blocked access of the main hoist. The fall of ground is approx. 250 feet down the sloped incline into the mine. MSHA was immediately notifed on their 1-800 number, and the Lexington Field Office.
A roof fall was discovered on the No 1 south panel, which was being pillared. The fall was discovered in the No 5 entry, just inby survey station 3219. The fall was an extension of a pillar fall which extended outby the break-line about 60'. The fall was 20' wide X approx 5' high. There were no injuries or equipment involved.

Two employees were entrapped on the elevator when it stopped in the shaft due to the cold conditions not allowing magnetic switches to opperate properly. When these switches do not ""make up"" the safety features of the elevator shut it down. the men were removed from the elevator without harm at 10:09 AM
A roof fall occured at an intersection where there was an intake/return overcast in 10th West at crosscut 11, # 6 entry. The roof fall was approximately 35 ft in length, 20 ft in width, and approximately 6 to 8 ft in height. The fall was at or above the anchorage zone and impeded travel in the return and intake entries. This roof fall was immediately report to MSHA.
During Sunday preshift prior to starting workweek, fall was found on the setup rooms on HeadGate#1 in #5 entry. No person underground when fall occurred.

#2 section intake, break 17, across from spad 3161 #7 entry. Person who found fall called another person and told him he found a roof fall. The size of the fall was approx 30' long X 25' wide X 5' high. Dangered off area and set timbers around fall.
The automatic operation of the service hoist tripped at approximately 12:50 am due to a failure in the cable gate switch. MSHA was notified by the emergency reporting hotline within the required time frame. There were no injuries or entrapments. The gate switch cable was repaired and the hoist could run automatic by approximately 2:00 am.

Emergency Escape hoist for Zone 7 became unoperational due to an malfunction of electrical power. We pulled the miners out of the Zone and put them into another Zone. There were no one injured or trapped during this time. Power was restored on 1/7/08 at 3:05 PM.
The roof above the last shield on the face flushed in and blocked egress off the tailgate end of the longwall. The tailgate entry remained intact with no roof failure involved in the entry.

Fall was 25' long x 20' wide and 8' high. no injuries resulted however aprox. 50' of belt line was lost. This fall will not be cleaned up. Plans are to reroute belt around fall.

The area was previously supported by 5 foot torque tension bolts and supplemental superbolts. Ventilation and travel were not affected. The fall was cribbed off.

A rock fall occurred in the intersection of the #10 entry in front of the D-seals at 3.5 North. The fall measured to be about 6 foot high, 19 foot wide, and 30 foot long. The fall was bolted with 4 foot fully grouted resin bolts. The fall was timbered on bolt side and will not be cleaned up.

An unintentional roof fall occurred on No. 10 belt, No. 1 entry, spad No. 8955. The fall measured 40' length, 20' width, and 5' in height. No injuries, no passenge of men were affected.
Hoistman reported that limit switch caused hoist to go down. Electricians were called to inspect hoist. They found the hoist was out of time due to declutching. Hoist was put back in time and returned to service. MSHA was called at 7:30 am reporting hoist out of service. MSHA was called at 12:30 pm that hoist had been returned to service at 11:00 am.
Roof fall was in the No. # entry at spad no. 8172. Fall area has been dangered off and will not be cleaned up at this time. Roof fall is 20' w x 20'l x 5' high.

Unintentional fall of mine roof located in the #4 entry on the 6 left panel at spad #351. This fall is approximately 22 ft long, 18 ft wide and 4 ft high and is 12,000 ft from any working section. This is a nuetral air course entry.

A fall of the roof occurred on #2 unit supply bolt, 2 crosscuts outby spad #66209, the fall measured 50 ftlx 19 ft w x 8 to 10 ft ft ht. No ventilation equipment or personell over affected.
Elevator was descending down the shaft and stopped at 163 feet. The computer fault could not be re-set and 30 minutes went by with one person in the elevator. Cause of this stoppage was ice formed on the governor rope due to cold weather and water in pit.
An unplanned roof fall occured 2 breaks inby spad # 4684. Ventilation was not affected, no equipment or persons were involved. Fall measured 105' in length, 18' width and 7' height. This area will not be rehabilitated and all entrances have been breakered off.
During the dismantling of the dragline, employees were cutting the cab loose from the machine with an oxygen torch. During this process some sparks were going into the cab. At the end of the shift all areas were checked for fire. None were found. After everyone had gone a fire started in the cab. Moderate damage to interior & flooring. No injuries.
While switching the hoist controls from manual to auto the hoist faulted. it faulted due to a malfunction in the hoist computers drive. The hard drive was changed out to the correct the problem.

On 01/01/08 an unintentional roof fall occurred on the 3rd South Submain, #6 entry, at #5 cross cut. The fall measured approximately 18' wide x 50' long, and over 10' thick. The area was bolted with 10' point anchor bolts. The area will be cleaned up as per the roof control plan.
Employee was walking behind haul truck when truck backed over him.
Two men were working atop an 80 ft silo attempting to reset the chain on a bucket elevator. One man was below and in phone contact with one of the two mechanics atop. As the call was ending, the assisting mechanic atop knocked a 'miner's bar' off the platform. The bar fell and hit the man below on the back of the head. The man died approx. three hours later in the hospital.
Employee somehow fell into the water. There were no witnesses so no details available.
Employee received a load of cement by the TXI scale person. After he received his load he pulled forward to a stationary rack designed for safe access to the top of each trailer. He closed all but 1 latch on the A trailer when he fell to the ground landing on his head. He was found at 1:30 pm lying on his back behind the second axle driver's side.
Employee exited the Hitachi Shovel and placed himself between the shovel and highwall. While on the ground between the shovel and highwall, the highwall sloughed and pinned the employee against the track of the machine resulting in a fatality.
Hauler backed over the outslope. The hauler traveled approximately 150 feet down the spoil pile. The operator was ejected from the cab of the hauler. Operator was a contract worker.

EE had right thumb caught between steel beams while replacing center section of floor following crusher repairs. As it was lowered into place a weld broke and the front section, consisting of a 12"" x 112"" C chanel and ass. handrail, swung down and crushed EE's right thumb against the structure. The piece then fell and hit the hand rails.
Employee's left thumb was caught between slide gate and slide plate.
Employees hand was caught between chain and rotating drill rod. Employee sustained a broken hand and forearm.
Employee was moving a tarp to cover a clinker pile. He claims he felt a sharp pain in his lower right abdomen. He was diagnosed with a hernia. He chose to have surgery after the job had ended on 3/28/08.
Strain to right groin while carrying a gearbox down a flight of stairs
EMPLOYEE CHECKING ALTERNATOR, REACHED TO REV ENGINE AND HAND WAS PULLED INTO ALTERNATOR, AMPUTATION LEFT INDEX FINGER.
EE stated that he was changing a fitting on a shuttle car and went to remove a crib block, when he caught the index finger on his left hand, crushing the tip. Approximately 1 inch of the finger tip was removed surgically. EE returned to work on the next scheduled shift.
The employee was building a steel lagging overcast. He attempted to raise one side of overcast with a pry-bar to remove crib blocking. The pry-bar slipped causing the steel lagging to drop onto the employee's thumb. The employee sustained an amputation of the tip of his thumb.
EE was riding personnel carrier when he went to adjust seat cushion,right hand slipped,he grabbed the side of the personnel carrier,the personnel carrier hit the mine floor, it smashed right ring finger and pinky finger against piece of rock. Cause of injury was awareness.Compliance with rules/regs not a factor,mining equip/systems not a factor, miner proficiency a factor.
The employee was starting to experience pain in his right groin and was diagnosed with a rupture of hernia as a result of repetitive trauma.
A bucket tooth feel off the Komatsu 500 loader and was wedged in the jaw crusher. Three crew members were working to free the tooth by timming the tooth with a cutting torch when the tooth shot out of the jaw striking the employee on the head
Miner had backed out of pillar line. MRS jack was being moved when a bolt head broke off letting a rock 67 x 72 x 6 fall and strike MRS operator in the lower back.
The employee stated he was going to pick up the continuous miner cable and felt a pain in his back. The employee has been going to the doctor for his back for five consecutive months. He was complaining about his back before starting to work.
Injured was putting a wrecked mantrip back on the track. Injured was using a jack and bar to lift the mantrip. The jack slipped causing the jack bar to strike injured in the nose.
Truck driver (785C Cat truck) was climbing the ladder to the cab, lost his grip falling to the ground. He suffered an open dislocation of his right elbow and fracture of his right femur.
Injured was walking back to his supply motor when he twisted his ankle.
Employee was moving miner cable and it got hung up with water line, and hurt his left shoulder **DID NOT START LOSING TIME UNTIL 2/28/08 DUE TO SURGERY**
While using a spline drive impact, the hose came off causing dirt from the surrounding area to blow into his eyes.
The employee, Roof Bolt Machine Operator, was installing an outside bolt in the roof bolt pattern when loose rib material fell and struck him on the inside bend of his right arm.
EE came out early & was cleaning belt sprays on the #6 truck dump belt.EE turned & the belt off and climbed up on the structure to access the spray bar,he was climbing he slipped & fell to the ground,twisting his left ankle & brusing his left heel.He reported the accident to his normal boss 3hrs later,he continued to work his shift.
While installing a roof bolt, the bolt bent. The EE placed his hand on the bolt and engaged the rotation level on the roof bolt machine. The roof bolt spun around, striking the EE on the right thumb.

Employee was pulling a 25 HP motor on a sloped catwalk. The motor started to slide backwards. The employee jerked on the motor. The motor pulled and strained his muscle in his right shoulder. Rest of shoulder was ordered.
While installing a roof bolt, a piece of draw rock fell, striking EE on the right hand, resulting in a fracture to his right little finger.
Removing jockey wires from MEM 362 empty. Tugging on wiring, got burning sensation in lower back.
EE was working as a roofbolter on the miner bolter. A piece of rock broke out of the roof, as he jumped out of the way of the falling rock he felt pain in his groin area. EE was diagnosed with a herina.
EE was walking on discharge chute. As he was climbing out his foot slipped causing lower back pain when he landed. The screen discharge chutes were on the ground while maintenance was being performed.
The worker stopped his truck to get his hard hat. He thought he put his truck in park. He went to the back of the pickup to retrieve his hard hat. The truck started to roll backward. When he attempted to stop the pickup it rolled over his ankle and came to rest against his side. He had a broken ankle.
Employee was performing cleaning duties on the roof of a warehouse. Accidentally the concrete steel rebar reinforced roof sections collapsed and a temporary worker standing on them fell the the floor below causing him minor injuries.
Walking on a bench, he stepped in a hole, hurting left ankle.
Employee slipped and fell on icy pathway while walking to plant. Employee suffered back pain and sought medical attention on 2/13/08. See attached explanation.
Holding bolter cable as it turned out of entry, as he moved out of the way, he twisted his knee. Miner did not miss work till 2/21/08.
EE was operating a dual head roof bolter with outside controls. He was installing his first bolt and after he swung the head to the center to install the second bolt, part of a loose brow fell and struck him on his back and legs.
Employee was walking near a valve with controls heated powdered material at the Preheater area. Accidentally the valve failed momentarily and allowed the heated material to escape causing him burns in his right arm.
Employee was at the shop assisting the mechanic with welding, (between the hours of 10:am and 1:30pm) suffered flash burn to eye, was treated that evening at emergency room, with ointment and pads for eyes, told to take next day off, but able to return to work 2/1/08. He called in sick for 2/1/08.
EE was walking under a piece of high line when a rock near the location where he was walking fell and struck the high line and knocked the high line loose. The line fell and struck ee causing him to fall. He tried to catch himself with right hand during the fall.
EE had performed maintenance on CM, had broken the chain, and was walking around the miner. He stepped on a slick rock, his left leg slipped, causing injury to the left knee.
EE was loading Thin Veneer. He stopped working because his arm was hurting him. He told his immediate supervisor that he pulled a muscle. He was sent to the hospital in Stamford for treatment.
The employee was attempting to position a piece of plate to cut up for scrap. The employee dropped the end of the plate and attempted to control the reflex bounce with his left foot. The plate struck the bottom of his foot causing the injury.

EE was hosing in the slope head house when the hose got caught on a chain, He jerked the hose to free it up and felt pain in his lower back and right hip.

While riding in a Brookville Mantrip, EE's left leg came into contact with the rib, causing fractures and an avulsion.
Employee was helping to pull the 350 cable along with two other members when he felt a pain in his groin area. He had continued to work until 7/7/08 when surgery was performed to repair the injured area.
Employee was filling a rental compressor with diesel. As he lifted the can to pour he lost his footing & fell, hurting his back.
The employee stepped onto a muddy truck wheel when he fell backwards on a crosstie and struck his back and neck causing a whiplash to his spine.
Miner was exiting the D7 Dozer he was operating, from the leftside of equipment. The Miner proceeded to step down from Dozer, when his left knee twisted. On 01/29/08, EE injured his left knee, but EE did not request any time off. At this time no lost time injury was reported, but on 2/19/08 at the request of his doctor, EE did not report to work.
3 EE's were bagging and palleting, the EE's were taking turns, one bags and the others pallet. EE was being trained on both bagging and how to pallet the #50 bags. Was no accident, EE starined his wrist.
Truck needed coal dumped off and driver got finger in tailgate.
Employee was walking and felt something in his left ankle pop. He stated he sprang his ankle while putting all his weight on it.
Employee was handling cable of roof bolter walking backwards when he fell over uneven surface and twisted his right knee.
Employee was using a knife to cut a cathead out of a box. The knife slipped and cut employee's right leg.
Employee slipped on muckpile while applying burlap to muckpile causing strain to shoulder. He continued to work as normal until Dr. restricted work activity on 2-18-2008.
Employee was cutting jack plates from the bottom of the revolving frame when the plate came loose and it bounced on the tub and hit the employee on the right leg just above the knee cutting the leg.
EE was walking in number 4 entry of number 4 section, when he stepped up onto a cable cross-over. While standing on the cross-over he lost his balance and slipped, causing him to fall.
EE was standing on the deck of a loader and was cleaning off the snow from the hood. His right foot slipped out from underneath him and he left shoulder fell against the hood of the loader.
Employee was pushing muck bucket over to rib. Employee fell down in water. Water got into boots. Chemical burns on both legs.
Injured had his hand touching a shuttle car and received a electrical shock which knocked him to the ground.

Crewman was walking down bilge when he either slipped or tripped and fell 2 feet down to bilge of dredge, landing on buttocks. Doctor reports bruise to buttocks.
While moving the miner, the employee was hit by the tail of the miner, when he tried to prevent the cable from sliding through the I hook on the tail of the miner. The tail of the miner hit the employee just below the rib cage.
Two EE's were sent to change some screens. One EE left to get tools and the injured EE decided to go ahead and begin pulling out old screens, without waiting for help. The EE felt a pull in his back. EE continued to work, but went out on lost time 4/15/2008 and accident became reportable.
Employee operating man basket lift in close to chute being dismantled. Man lift became hung on metal angle iron. Employee had his head between the control station and chute when it dislodged catching employee's head.

While traveling on the haulroad, the axle housing for the right rear tires separated and the end dump dropped to the road surface. Employee reported a stiff neck on 1/29/08 and did not report for his scheduled shift.
Employee was walking backwards, pulling a water hose and tripped on a pipe. Employee fell catching herself with left hand. Employee sustained a sprain to left wrist.
EE was removing a lid from dust collector, slipped on pipes below his feet and twisted lower back.
Employee stooped down to pick up a box weighing approximatley 20 lbs. As he raised from stooped position, twisted slightly and felt a pain in his lower back.
While ""rodding"" mill 3 feed tank, employee strained right shoulder.
Injured was watching the helper's side operator re-attach the hose to his drill head on a Fletcher DDR roof bolter. The helper raised the mast to attach the hose and was unaware of the injured, who was standing on the outside of the machine, placing his hand on the mast. After attaching the hose the mast was dropped and the injured fingers were caught in the mast.
Employee was making the fireboss run through the mine, while traveling in the mantrip, he hit a rock that had fallen on the No. 3 Belt. The rock turned into the operator's compartment striking employee in the right side.
Employee stated that he was dusting a beltline when the dust hose ruptured causing the nipple end to strike his left ankle. First lost workday did not occur until 11/24/08 due to surgery.
EE was moving bolt cable using his right foot, when the cable tightened and knocked him off of his feet, causing him to break his ankle.
Operator was moving drill parallel to a cutbank when the material on the crest gaveway. Drill tipped over on it's side and fell into trench. Operator was ejected out of the drill through the window and came to rest when he hit the drill mast. Operator received two broken ribs.
On Saturday 1-26-08 scratched elbow on screen support tower. Did not report scratch on day of incident On following Wednesday he said elbow was stiff and arm was getting red. On Thursday elbow was swollen, stiff and arm red. Sent worker to emergency room. On Friday 2-1-08 went back to Emergency room because arm was worse and was admitted to Hospital.
Employee notified management on 1/29/08 that on 1/26/08 while operating a rock truck, the loader bucket dropped and hit the truck bed. Employee stated that this hurt his head, neck, and back.
EE climbed down from dozer and he forgot his cell phone, as he stepped on push beam to climb back up, his foot slipped and his leg went between push beam and track frame and he twisted his right knee.
Employee was installing angle iron in an overhead location. Piece had been positioned with clamps and was to be tack-welded in place prior to removing the clamps. Approx. 20 seconds after removing the clamps the tack welds failed allowing it to fall approx. 3 feet and strike the employee on the right hand causing a crushing laceration to the back of his hand.
Running scoop and drove across concrete block. The jar put strain on lower back. Note: Lost time as of 1/31/08.
Employee was working in 4cy5. At 10:30 pm a large piece of coating fell from the upper walls of 4cy5. The coating fell on his head and left leg.
EE was in the process of tearing out old refractory inside the preheater tower using an air powered chipping gun (rivet buster). As EE was pushing the rivet buster into the refractory the bit broke causing his hand to slip and his left index finger to be caught between the rivet buster and the wall.
EE twisted L knee as he walked on frozen ground to restroom while doing maintenance in plant area. Initial diagnosis was bruise and strain to knee; full duty, no restrictions. On 5/19 EE complained of occasional soreness and stiffness to knee. Sent to orthopedic surgeon; MRI determined a tear of posterior of medial meniscus. Surgery scheduled fo June 8th to repair.
Employee bent over to move a short section of steel beam.
EMPLOYEE WELDING AND FOREIGN MATERIAL BLEW INTO LEFT EYE.
Employee was lifting the lid to the cable reel compartment; while doing so he felt a pain in his right lower abdomen and fell to the floor.
Caustic loader was loading 50% caustic into truck tank; hose was plugged with frozen caustic. Operator closed valves & disconnected hose. Hose was under pressure & sprayed operator on L ear, L neck, L shoulder, L back area, L chest & L eye.
Injured was closing machine doors and had closed half of door when the other half closed catching injured between doors.
Individual was working on #3 conveyor, failed to lock-out and the operator inadvertently turned the belt on pinning the employee between head pulley and stone-box. Result: damaged colon requiring surgery to repair.
Employee was operating product bagging machine - when he pulled the bag dislodge handle with his right hand, he felt a pain in his right shoulder/neck area. He continued to work until the pain worsened and he sought doctor's care at 2:30 pm.
EE was pulling a scoop with another scoop and chain, chain slipped and hit EE.
Employee was dismounting a haul truck. Employee struck his left pinky finger on the handrail of the haul truck.
Three EE's were installing a new crusher guard. They were using a come-a-long to pull the guard up and into place. The guard was sitting to far from center and was going to slide back. So the EE got behind the guard and gave it a shove to center it up. In the process of pushing, he felt severe pain in his lower back. MSHA lost time accident 1/28/08.
Slipped on ice, injury to right ankle.
Injured was riding in an open mantrip when a piece of rock fell from the roof and struck the injured on the neck and shoulder.
Employee was climbing down the ladder and slipped on the bottom step. Caught himself on the handrail resulting in hurting his back.
Employee felt muscle tightness torquing head bolts on rear engine of 9017 shovel. Felt tightness and soreness in arm next morning. He received diagnosis on 7/20 tendinitis and will start physical therapy next week for several weeks. He is also missing time as of 7/24/08.
EE was bending a 6' bendable resin roof bolt and slipped on pavement and injured right shoulder while leaning forward into the roof bolt.
Employee began missing work from an injury that occurred on 1/24/2008. He was rockdust and felt pain in lower back.
Employee stated he dismounted D10 to use bathroom and was climbing back on the tractor when he slipped injuring his knee. He had knee problems prior to accident.
Miner cable struck EE's head, shoulder and thumb while lowering it from a hanger. Evidence obtained from a compensation review on 6/27/08 dictated recording the incident.
He was jacking the track back on the trackhoe with a come-a-long and the chain broke on the come-a-long. The track fell on his left leg hitting him on the leg about the shin area.
Walking to manbus, employee stepped in a hole with left foot, he hyperextended his right foot and felt pain in the big toe.
EE was walking down launch on highwall miner, slipped on ice and fell to ground approximately 4 1/2 feet hitting left knee, then was transported to hospital.
Injured was cutting a piece of chain with bolt cutters when a piece of metal flew off and struck the injured in the face. The laceration required 4 stitches.
Inspecting pockets on pocket feeder, as he was removing the cover, the cover fell. As he tried to catch it with his left hand, he lost his balance and his middle finger on right hand came in contact with the moving feeder. Cutting off bottom tip of finger.
While bonding a intake stopping he used his bare hands to put b-bond on a block stopping, he was offered a pair of gloves. He choose not to use them.
Employee was walking past the scoop when he stepped on a pallet. The pallet broke and he twisted his knee. Employee started missing 01-28-08.
EE was pulling a pump out of a drill when he experienced pain in his abdomin.
IW felt pain in shoulder and neck when tightening a vise on a grease pump.
EE was stepping off of a ladder, when his foot slipped on the edge of a trough in the floor, causing him to fall and strain his left knee.
Employee was on a break sitting on a work bench. Another employee pulled on the employee's legs causing the the employee to slide to the edge of the work bench. The employee reached down with her hands to prevent her from falling to the floor causing her to strain her lower back and hip.
Chipping ice from conveyor belt, he slipped and struck left hand between belt and pry bar.
There was a bounce on the logwall face at the tailgate at shield 123. EE was operating the tailgate cutter drum and was pinned between the bounce guard and the sheild resulting in ee being bru ise and put on light duty for 3 weeks
Employees were standing on a loader bucket trying to free a conveyor head pully. The loader operator in error dumped the men off pinning one between the bucket and the conveyor. The injured employee was transported to the hospital and found to have no broken bones or internal injuries. Contributing factors were the cold wet weather and disregard of company policy.
Employee was lifting a drill bit into a truck when he felt pain in neck and back.
Employee was riding mantrip looking for a water leak on belt, when he hit his head on the mine roof.
Employee was packing a rock over the stage loader to put on the pan. He slipped and fell. Landed on his stomach pinching hand between rock and panline. Finger on left hand was cut requiring five stitches.
Employee was climbing down the ladder of the 777B Rock Truck to fuel up at the end of his shift, when he felt a pain in his shoulder. Employee shift started on 01/21/2008. Employee continued to work until he had surgery on the shoulder on June 20, 2008.
While loading rail onto rail car, section of 60 lb rail fell onto right foot, causing injury. Taken to ER, X-Ray positive.
Employee was dismounting from an R-60 Euclid Truck on grase pad. Left foot slipped thru bottom swing step, causing employee to fall, bruising calf area of left leg.
Employee was lifting a 50 lb bag of material when he felt a pain in his right shoulder. Possible muscle strain in the right upper arm and shoulder.
The employee was having to walk in deep mud while working in the field. He stepped in a hole and felt that he twisted his left knee while trying to get out.
Employee was disconnecting a hydroblasting shotgun under a tank containing sodium hydroxide. An external pipe to the tank was clogged, when suddenly the plug was dislodged, resulting in the sodium hydroxide to engulf the employee's head, face, neck and sholders.
Employee was priming blastholes to be loaded that day. While getting ready to prime one hole he noticed a rock on the edge of the hole. He kneeled down to move the rock before he primed the hole, and when he stood up he twisted his knee. Employee was taken to the emergency room where he was told to take 3 days off.
Employee slipped while exiting truck and injured back on the 22nd. He did NOT seek medical attention or miss work. On the 24th, his vehicle slid into berm in snow, reaggravating injury from the 22nd. He did not seek medical attention until the 28th, at which time he was off until 2/2/08.
Fixing tear in conveyor belt left hand got caught - smashing it. Employee has been treated at Union County General Hospital.

Riding mantrip, his hard hat got caught on water lines pulling head & shoulders into roof.
When preparing to enter the mine, the excellerator stuck on the buggy while in reverse. The 3 men on board had to evacuate the buggy as it was going to hit a concrete structure and possibly cause a serious or fatal injury.
Injured was using a sledge hammer to drive a coupling back onto a motor shaft. As injured swung the hammer he felt pain in his lower back. (Note; company was not notified injured was off work due to this injury until 02/27/08)
Driver pulled out of pit, started to tarp his load when tarp became stuck. Driver got out of truck, walked around passenger side & tried to push tarp back and his right wrist popped. After his wrist popped he walked around to driver side and climbed up side of truck bed to see if tarp was caught on something.
Employee was using torch to cut a broken place out of a hydraulic cylinder, when the oil in the cylinder ignited and flames shot underneath his helmet, burning his face.
Employee was cutting a tree and it got lodged. In an attempt to get the tree on down, it broke loose striking the employee on the right shoulder causing a fracture.
Injured reported to work sick at his stomach and feeling nauseated. Injured was in load out when he said he felt sick and stepped outside & threw up. At this time he got light headed, passed out & fell backwards striking his head on metal step causing laceration to back of head.
EE was walking and did not notice he was standing on ice, the ice was covered with sand. The ice broke and he went down to his shins and jarred his back.
Employee reported on 1/21/08 that he had low back pain which he felt was the result of shoveling coal over the last few months. No one specific incident was identified to cause the back pain. The first missed work day was 1/25/2008 and he was released for full duty 2/1/08.
miner picked up and was moving a water duke when he felt a pain in his left shoulder.
Employee was closing doors on the rail cars when he slipped twisting his ankle resulting in a fracture in his ankle.
Ee slipped and fell on stairwell at washer. There was ice on the step. Strains and bruises to back, knee and elbow.

Unloading hydrate lime from bulk tank trailer into lime storage silo. Silo filled and transfer hose was unhooked before all pressure was drained causing lime to gush out of hose and onto employee resulting in lime getting in eyes even with safety glasses on.
Jackleg started falling, pushing him off the power pack. He fell and broke his left ankle, requiring surgery.
He was walking to the longwall face when he felt pain in his right foot. He continued to work at his regular duties and went to a physician on 1/31/08. He was diagnosed with a stress fracture to the right ankle. Please consider this occupational illness. I have included information about stress fractures for your review. He first day off work was Monday feb. 4 2008.
Lifting a bucket of gear lube to service a feeder crusher. Felt pain in lower back. (Lost time as of 1-21-08).
On 1/19/2008 the employee was stepping down off the stageloader at the longwall. He felt something pop in his left knee. He has continued to work and started missing time on 5/9/2008 to have surgery on the left knee.
DID NOT START LOSING TIME UNTIL 7/31/2008. Bent over picking up slider curtain and 4x4 timber fell on his right shoulder.
He was loading buggy when a piece of rock 3' x 1 1/2' x 3"" slid from off the top of buggy and hit him on top of his left foot.
The miner was walking on snow-covered ground. He slipped on ice covered in snow and fell on his right hip. He reported the accident when he decided to go to the doctor on 1-29-08. The doctor said that the injury was a bruise & strained ligament. Three months later the miner still had pain in the hip. He visited another doctor who diagnosed a hernia.
Drilling a harness clamp when glove became caught in machine. Injured left hand when pulling hand free. Employee was seen in ER. Fracture.
EE was using a one inch imact gun in the removal of finish mill liners in the 1st compartment of the mill. Some small balls came back into the first compartment. 7/8"" diameter. As he was lifting the imact gun the EE slipped on the balls, striking his left knee.
Employee was climbing out of scoop when he said his foot got caught on scoop causing him to fall out on ground, twisting his back and causing his right leg to go numb. Employee started missing 01-21-08.
While closing the west gate into the carbon columns the employee slipped on the ice and strained his lower back and left knee At first it was Untreated later he went to the doctor 3/27/08 on 1/20/09 had surgery on left Knee
Going across parking lot, jumped across a ditch & landed on side of left foot & felt it pop.
Member was using torch to remove lock covers off a tool box. He stated that he had completed the task, turned torch off & layed them down. As he turned around he felt his right side getting hot. Member received 2nd & 3rd degree burns on right side of abdomen.
Member was driving water truck down Laxare haulroad. Member said his brakes were not holding and he drove the truck into a sump. Member complained of pain in his right hip.
Employee was climbing on a road grader and when he went to pull himself up he felt a pop in his right shoulder and he felt a pull in his muscle. He went for a follow up visit to the Doctor on 02/14/08 and was put on sedentary duty and was scheduled for surgery. Became a lost time on 02/20/08.
The employee was assisting a bolter operator by making up his bolts. He was positioned to hand a bolt to the operator when the employee was struck in the side of the lower leg by a rock. The employee sustained a hyper-extended knee causing fractures of the ankle and knee plus possible ligament damage.
As the operator was returning from the ash pit in a Komatsu 330M end dump, he met a fuel truck and slid off the road. The roads were wet from rain, and the road way is narrow. The operator called the supervisor on shift to assist in bringing a cable choker to pull the truck out with. As the operator was pulling the choker into place to hook up to the truck, he strained his back.
Employee was walking into the main entrance from outside and slipped on the floor-twisting his right knee. He was evaluated by a physician and on 4/10/08 had surgery, resulting in lost time.
While shoveling in draw off tunnel. Back pain in lower front ribs.
Pulling on a chute - broke free and smashed his left hand. Cut hand and got some stitches.
DID NOT START LOSING TIME UNTIL 2/12/09. While unloading supplies with lo-trac, drove into hole and jarred his neck. Requiring surgery on neck.
EE was walking around on upper bench and fell into a hole.
The employee's assistance was needed to remove debris that was clogging the crusher, not allowing it to work properly. In the process of clearing the jam, a 25lb rock rolled down onto his hand pinning his fingers between the rocks. He realized his finger was injured and was taken to the emergency room.
Employee states as he stepped off the monorail skid into some deep mud, he stepped on a rock or chunk of coal and twisted his left ankle.
The man was lifting belt structure when he felt pain in his lower back.
Employee stated he was exiting the mantrip; when he turned he felt pain in his right knee. Doctor removed him from work on 1-24-2008.
While working on the continuous miner he twisted around and felt a pain in his leg.
Employee was dismounting 988 Loader and twisted his back.
EE was lifting chip tray, as he lifted the tray he felt his arm pop. The weight of the ship was 10-15 pounds,
Crane operator was picking up crusher part and attempting to place in loader bucker. When injured employee tried to re-adjust the chain strap, the crusher part shifted and pinched his left hand between the part and the loader bucket.
Miner states he injured left knee while climbing stairs. Did not notify management until 02/05/2008. Injury disputed by Workman's Compensation due to pre-existing problems with knee. Claim upheld on 04/14/2008 and IMV Nevada notified on 04/21/2008. Accident Report filed 04/21/2008.
Employee was plastering a stopping and got plaster in his eyes.
Coming down the ladder on a 785 B rock truck and the steps were bent under the truck. He missed the bottom steps and stretched his back.
When climbing off of a float trailer, he jumped off of the back tire, felt a sharp pain in butt and leg. Approximately 2.5' from tire to ground.
Employee was adjusting forks on the forklift. The pin was not secured and the mast fell on him.
Walking around tailpiece and knee gave out on him. Fell to ground, said knee was hurting.
A rock was lodged in the crusher. A chain was put on rock to pull it out. Loader operator asked injured if all chains were on. He replied 2 chains were in loader bucket. Loder operator didn't know injured had climbed into bucket. He lifted loader bucket Appx 8 feet and began to dump chains out. Injured fell out of bucket landing on feet & falling down. Chipped heel bone.
Pinched his thumb between two wear-plates while repairing chutes. The employee did not seek medical attention until July 8, 2008 after pain developed where the thumbnail is growing.
While working to unplug the #5 dust collector hopper, material broke loose from the upper section of the hopper and leaked out the access door, causing material to get into the employees eyes and mouth. Material irritated the lining of the employees mouth and eyes.
Employee was getting off his loader, as he was coming down ladder his slipped on a step a fell to the ground landing on his behind.
Employee was bolting top. When he started his drill steel, a piece of rock broke out from the top and hit him in the nose.
Member was working on outby construction at the #7 North Head. Member was descending a step ladder and stumbled, twisting right knee. Worked up until 4/4/08. Ortopedic sugern will scope knee.
Ran over rock in road while driving a golf cart causing the steering wheel to turn suddenly, twisting his wrist and side.
Operating a cutting machine and his coat caught a roof bolt; jerking his head back, straining neck.
Driver failed to lower bed on 10 wheel dump truck, drove approximately 1/4 mile and struck bridge which removed bed from truck. He was taken by ambulance to hospital for evaluation then transferred to Wichita Hospital for further treatment.
Employee states he was mixing mud next to the hopper, when the other helper jumped into the water truck and began to drive away with a hose still attached (helper forgot to unhook hose) to the pump. The pump began to drag and came towards employee, who jumped out of the way and landed on the ground and felt pain in his knee.
Employee exited her truck (5997) and went to shut the oiler off at the top of the Gencor (#6303) and slipped and twisted her knee. Knee strain w/small tear.
Employee was pushing buckets in on the empty side of the tramway, when the dispatcher released two buckets instead of one. Employee tried to stop the extra bucket by grabbing ahold of it and slid about 8 feet, trying to hold it back. Employee complained of shoulder/arm pain and sought medical attention. Employee has been off work since date of injury.
Employee was climbing ladder into haul truck and his knee ""gave way"". There was no cause, did not slip or bump knee. Employee has torn medial meniscus of the left knee requiring surgery. This is a reportable lost time accident.
Gas leaking from pipe ignited and caused burns on head and arms.
Employee spent 5 minutes or so sweeping floors and later, an hour or so, lifted som boxes. She never reported any pain or symptoms on the day of the ""injury."" She called the next day and said she thought she hurt her back sweeping.
Employee was drilling hole for roof bolt, steel fell striking his hand.
Walking from mantrip to working place and stepped on uneven bottom, twisting right ankle. He worked his next scheduled shift, but began losing time on 1/18/08.
Employee was walking on the section, when he stepped in a hole, twisting his right knee. Reportable lost time injury as of 2/2/08.
Changing oil in drill rig, employee walked around front of drill, slipped on ice and twisted ankle. Employee heard ankle snap and was/is unable to put weight on it.

Employee was stepping down from a ledge and his foot went into about 10 inches of mud. While trying to take a step he pulled his knee trying to dislodge his foot. This event was not reported for 3 days.

Employee was helping carry a calrod heater, walking backwards between screen deck and bonnett and his foot slipped off end of plywood and went into lime burning chute.
Employee was placing a large piece of C channel in place and felt a pain in his lower back. Off work as of 2/8/08.

EE working on drive and went to place 4ftX8ft. guard back up. He placed one end of the guard back on the pole bracket and then went to lift the opposite end. He lifted the guard up, the other end that was set slipped off the bracket and caused him to bring his guard up to the back and then straight down. Pinched his thumb and index finger against the pole bracket, fracture.
Supply motor was pulling the car up and the supply car caught the back of his right foot. It twisted his ankle and chipped a small bone in the heel.
The employee was splitting rock with a hammer and smashed his thumb.
While loading a miner cable in #2 entry. Hit lower left leg on a bucket scoop and twisted right wrist when putting cable into bucket.(Lost time as of 1-21-08).
Employee sprained left ankle when he fell over metal laying on ground.

Employee was standing on rear bumper of roof bolter replacing bolting materials when pie pans fell off of bolter striking foot. Employee missed work on 01/16/08 due to swelling/pain in left foot.
Roof bolter was being positioned to install 4' resin bolts & mats. As the machine approached to final 2 rows of bolts previously installed so as to begin bolting the new cut, a piece of rock fell from between the bolts striking the victim on the back. Rock Appx 12"" w, 3' long. ATRS & post jacks not yet deployed as the roof bolter had not yet reached it's beginning position.
EE was assisting miners in making belt splice at 3-head.EE was pounding nails into splice w/ hammer,hammer struck his left thumb.EE sustained dislocated thumb and needed 2 sutures.EE was released to RTW on 1/16/08.EE worked part of his shift on 1/16/08.EE went back to doctor and is not scheduled to RTW until 1/22/08.Miner proficency and awareness were factors in incident.
While guiding a pin into the hole with his knee, the employee felt his knee pop and pain shot down his leg.
Employee was operating a Personnel Carrier when he ran over a rock in the roadway, causing the steering wheel to suddenly turn, striking his right hand. The employee sought medical treatment for this injury at a later late & reported to Premier Elkhorn Coal Co. on 2/6/08, that he had suffered a fracture as a result of the accident that occurred on 1/14/08.
Employee was assisting with extinguishing a fire that had occurred around the collector rings on the 495 Bucyrus shovel and as a result was burned.
EE's cleaning up @ the end of shift,Tools were placed in box & lowered to ground floor of mill bldg with overhead hoist. Hoist opr. was raising the hook so the hoist could be parked. EE's gathered at the box, hoist did not stop and cable broke, block & hook fell stricking EE on hard hat. Possible skull fracture.
Cutting cable with torch, burned left knee, 2nd degree burn.
EE was walking on off side of scoop, when he stepped on a berm of a rut, causing him to loose his footing. EE completed his shift and drove himself to the hospital. Had X-rays which showed no evidence of a fracture and was referred to a specialist who diagnosed him with a severe sprain and removed him from work.
Employee slipped while tightening a bolt with a ratchet and strained his shoulder. This employee worked for several weeks then went off work and had surgery. Date noted above was first report of injury. Difference of date of injury and date of report was pending a medical report from physician and acceptance as in industrial injury.
Employee was standing on east side of TRM II mud line chipping out a plug of frozen mud. The frozen plug got a hole in it and cut loose. An unknown amount of pressure forced the frozen plug out along with mud hitting his head/face.
Employee was assisting with extinguishing a fire that had occurred around the collector rings on the 495 Bucyrus shovel and as a result inhaled smoke.
PUTTING ON RUBBER BOOTS TO BEGIN WORK OUTSIDE.
The individual was working on the No. 4 conveyor belt take-up unit when rib rolled and knocked him to the ground. This resulted in fracture of his lower right leg.
Employee was reaching down in a chute to clean and overstretched.
Employee was kicking dirt over down lines when he felt a pain in his right knee. He reported the pain but did not seek medical attention until 1/18/08 as the pain increased.
He was standing on a small ladder, tightening a roof truss, when a piece of rock began to fall. As he stepped down from the ladder he twisted his left ankle causing a strain to the left ankle.
Removing track hydrant, turning valve off and valve came in two. Sprung and hit hand.
Employee was tramming scoop down entry. Fly board caught him in canopy of scoop and pinned his back against the scoop canopy, scraping his shoulder blades & pulling back muscles.
Employee inserted 2nd steel while roof bolting and raising boom, steel bowed then popped into chuck causing steel to strike right hand. He sustained contusion and swelling to the right hand. He missed work on 1/14/08
Employee was getting out of Bulk truck. As he went to get down he lost his grip and fell to the ground. Employee went to emergency room and was told he was ok. Still suffering from pain 3 days later he went to doctor who found a fractured rib. Doctor released him to work. Still in pain 2 weeks later he went to doctor on 1-29-08 and was told to take a week off.
Employee was waiting for his shuttle car to be loaded when a foreign object hit him in the eye.
While backing up the continuous miner after cleaning up some rock in the No. 1 entry the miner operator caught his right leg between the cable standoff and the rib. The operator sustained a fracture to his right femur.
Employee was driving 4-wheeler past a timber and his hand was caught between the timber and his ride.
Employee had a utlity knife, trimming on a roll of 48"" belt. Knife slipped, cutting right arm. Cut was about 2"" to 2 1/2"" long.
While employee was operating the miner, he backed up and caught his upper right leg between cable arm and rib.
While mounting dozer, employee felt a ""pop"" in his shoulder. He was diagnosed with a rotator cuff injury. NOTE: Employee did not begin missing work until 4/16/08.
Employee was operating the fletcher roof drill he let the drill head down on his right ankle.
Employee slipped on stair while walking out of screen tower. He was holding onto the handrail and pulled his right shoulder. Diagnosed with shoulder sprain and then with a torn rotator cuff. Surgery on August 25, 2008.
Employee was walking down side of a pile of overburden when his right ankle folded under his weight, breaking bone in ankle.
Employee stated he was mounting a dozer when his right foot slipped, causing his right knee to receive the full impact of his weight. Employee stated that he felt his left knee ""pop"".
On 1/11/08 the employee was working to re-rail an off track mine car. As he was prying the wheel hub, the car accidently moved causing the bar he was using to jar his left arm & shoulder. He continued to work until 2/29/08 when he started missing work due to pain in left shoulder.
Employee drove through pot hole. Steering wheel spun and struck finger.
The employee was working in the stamler area at the raw end of the plant and while putting the cleaners in the stamler got his finger caught in a pinch point.
Employee was placing a battery on charge. As he lifted the lid it fell striking his right hand.
Injured employee was crawling through crosscut, when a piece of draw rock fell on him.
Injured had been shoveling on the slope belt. Injured was walking up the 18 degree incline slope when his foot slipped causing him to fall to his knees. The fall jarred his back resulting in a back strain.
Injured employee was installing outside roof bolt, when a piece of roof rock fell on his left hand that was placed on top of the machine, resulting in a fracture.
Employee stated he was walking toward the face when he slipped on a flat slick rock. He lost his balance and recovered from falling but twisted his knee. He continued to work until 9/22/2008 when he was taken off to have surgery.
Employee was stacking 50# bags of product in the presswell when he felt a spasm in his upper back. He finished the work period, went to break, then went back to work to finish loading the pallet. He then notified his supervisor of the pain at approximately 10:55 am.
Employee was using a skilsaw to cut mine timbers. The saw kicked back and cut his right thigh.
Employee was struck by a piece of roof coal that fell from between the roof straps.
Left ring finger on left hand, putting rock dust in machine. Guard was loose allowing finger to hit auger, had to have 13 stitches in finger.
Using 70lb sledge hammer inside 5' diameter mill, felt pull in lower abdomen. Did not seek treatment until 3/07/2008; Diagnosis: inguinal hernia. Did not recieve treatment until 3/18/2008.
On 1-10-08 at 10:00 PM, a packaging operator was clearing a jam at the second level of the line 3 palletizer. The operator was attempting to carry 3, 10 pound bags of product down the steps from the second level. With both hands full, the operator missed the last step of the stairs and fell to the floor. The operator suffered a sprain / contusions on his right knee, and leg.
Employee slipped in mud in parking lot and tried to catch himself as he fell causing his right elbow to become dislocated. Physician took him off work effective 1/17/08
Employee was throwing a track switch outside and twisted his right knee.
Injury initially occurred on 1/10/08 when employee injured right arm while removing cheek plate bolt from jaw crusher. Did not become reportable until 5/28/08 when the employee had surgery on torn bicep muscle in right arm.
Slipped on ice going into truck shop building.
Employee lifted a pallet and radiator using a forklift. When he noticed the radiator was not square on pallet, he stops forklift and straightens radiator by lifting one corner. As he lifts, he felt a ""pop"" in his right bicep.
Employee was stacking structure into a supply car and a piece fell and cut his finger.
Employee was removing tailgate from a John Deere articulating truck. He had secured a chain to the tailgate and an excavator to take the pressure off of the pin holding the gate. He decided to cut the bolt that holds the pin. After cutting the bolt the tailgate swung, striking the employee in the back on his right side just above his hip.
Employee stated that while loading rails he was helping move a timber when his coworker tripped, causing him to fall. The timber struck his right ankle causing a contusion/bruise.
EE alleges that he experienced back pain while he was being loaded. He was taken to the ER and taken off work. ....Under investigation....
Employee fell after stepping off roadbase catwalk; looked to see how area around tail section had been washed away to allow unit to turn, as he hopped down with his left foot, ground gave away. He turned to his right trying not to fall on conveyor, he fell to the ground on left side. Said that his left lower back was in pain.
On 1/10/08, 3:35pm, employee reported that his left knee was bothering him. He did not know how or when he hurt his knee, but it started bothering him when he was driving the forklift around 1:00pm the previous day. He wanted to give it a few days to see if it got better. On 1/15/08 he wanted to go to the doctor. The doctor's diagnosis was a sprained knee.
Employee was climbing down ladder in raise, rocks started falling the employee scurried down to the bottom and jumped out of way landing wrong.
During reassembly of new equipment, EE hurt his knee.
Employee was sitting on rear of jumbo drill as it was drilling holes at the face. He jumped off the jumbo a vertical distance of about one foot to get fuel for the jumbo and landed on the side of his right foot. The right foot became immediately painful, but he finished out his shift to 4:00 pm.
The employee stated he was pouring a five gallon power tran tractor hydraulic fluid in the tank when he lost his footing. The employee stated that he felt a sharp pain in his lower back as his foot slipped and stepped into an opening between the frame and battery.
Employee was handling bullhose while mucking. Rock fell down shaft and hit right little finger. Contusion of finger.
Employee was bolting top and twisted his left knee.
Dropped off in haulway he was running a scoop. Batteries on scoop hit solid. When he hit the hole, it bounced him up and down.
Employee was checking oil on D10N when he lost his footing during heavy rain. The grab rail caused him to twist possibly causing strain or back injury.
Employee lifted a barrel and felt pain to his umbilical area.
While hanging a line curtain in the #1 entry the employee was slightly bumped with the battery end of a battery scoop causing a contusion to the right leg and hip.
Stepped up on step stool when installing windshield. Step stool slid out from under my feet. Caught self from falling and hurt back.
While repairing the cable trac on the Novamac drill, operator fell approximately 7' to the ground. Strap broke causing operator to lose his balance.
#5 Section #2 Heading. Upon investigation employee was dating up in #2 Heading when scoop driver came through the curtain and misjudged his turn into left break, catching employee with battery end of the scoop in some gob on left side of heading area.
Going in on mantrip, scoop bucket. Stopped to get out at head drive, getting out of scoop bucket, had hand on side of bucket and a personnel buggy rolled into scoop bucket. A pinched finger and broken finger.
Unloading pipe from back of truck, pipe came back and hit him, bending him backwards.
Employee was changing a belt scraper; scraper fell catching his left hand between scraper and belt frame.
The employee was assisting in the removal of a wheel unit from a battery coal hauler. The unit was suspended on the forks of a mini-trak by a chain. The wheel unit rotated striking the employee's right knee. The accident occurred on 1-8-08. The EE did not have trouble with the injury until 1-13-08. He was seen by Dr. on 1-14-08. Same knee operated on in 2003, different reason.
EE was installing perminant roof support, when a piece of draw rock fell, striking him on the left foot. EE finished the shift, however later on that evening, he went to a local ER where X-rays revieled he had a fractur to the bone directly behind his big toe.
Employee originally stepped awkwardly in a dip in the ground on 1/8/08 hurting his right knee. Employee continued to work. He then states that he re-injured the same knee on 2/16/09. He continued to work. His knee has evidently worsened and he had knee surgery on 3/27/09. Employee has since returned to work, seated duty only, on 4/15/09.
Employee was placing drill steel in the chuck on pinner head. A piece of rock slid down the drill steel catching his thumb between the rock and the pinner head. Went to doctor on 1/8/08. Started losing time on 1-9-08.
Injured employee working fell from conveyor belt and hit his head.
The employee was operating equipment and went to exit the equipment by climbing down the ladder. He was using 3 points of contact (2 feet, one hand) when his feet slipped(due to mud and temps). He caught himelf with his hand, pulling on his shoulder. He completed the shift.
Employee strained left shoulder by lever jolting while raising truck body.
Information received January 24, 2008 that the employee saw a Doctor on January 22, 2008 and is to be off unitl January 29, 2008. The employee has stated he was using a rachet to loosen a bolt on a coal feeder on the No. 1 section and strained his lower back.
The employee stated he was loading belt on two jack cars and he was pulling on the belt on the cars as they were pulling the car. He felt a pain in his lower back and leg.
Employee was assisting in relocating a section of pit pump cable and stepped into a hole as he was pulling the cable. Employee stated that he felt his left knee hyperextend. Employee was wearing all PPE required at the time.
Anfo rig was being brought to the shop for repairs. An engine malfunction caused the machine to start smoking. Smoke started to fill the drift. 5 miners were evacuated from the drift.
Bag burst blowing lime dust in the right eye of the employee.
Miner cleaning windows on haul truck. He slipped on a truck step and twisted his knee.
The man was pushing on a wrench to loosen a tight bolt when he felt a pain in his side.
Injured was releasing the stuck door on the gravel car when it broke loose and handle struck him in the mouth. He lost several teeth; several lacerations occurred.
While hanging a miner cable, EE was injured, when he fell off of a ladder.
EE was complaining of lower back pain at start of shift. By 6 PM he stated that he could not run shuttle car any longer due to pain and asked to be brought outside.
Employee was removing a hydraulic cooler line inside frame at right rear of loader. Ackward position hard to manuever hydraulic line caused shoulder strain. No medical treatment at the time. Employee subsequently had an appointment at clinic doctor recommended surgery when appropriate. Surgery scheduled 7/9/08.
As EE was doing his normal routine roof bolting, he stepped on a rock, slipped off it and turned his ankle.
Miner was tightening bolts on a cover. The socket slipped and the miner struck his hand on the top of the water spray bar.
Was receiving training on bobcat. Jumped out of bobcat as equipment was accidentally started and bucket began to lift.
Employee was using a wrench to remove bolts from a fan door. One of the bolts overhead was hard to remove, employee used both hands on wrench and when pulling downward, heard something ""pop"".
Miner was clean-up in the pre-heater tower. He was wearing a face shield which had accumlated some dust on the lip of the shield. When he lifted his face shield up, some dust dropped in his eyes. He suffered a corneal abrasion of his left eye.
Roof bolters arrived at bolter. Tram side operator got in tram deck to tram bolter. The opposite side bolter went to step over the cable when the operator turned the cable reel on. This caused him to fall which injured his leg.
Injured was walking from power center located at 4 west, when his foot slipped, resulting in a twisted (sprain) right knee.
