Accident type, without injuries

A roof fall was found during mine exam at cross cut 74 entry #4 5R MS. Fall was in a four way intersection that had been mined October 2004 and supported with 8' full resin pins. Fall was 40' long x 30' wide & 10' high. No injury or property damage resulted. There are no plans to clean this fall up.

Fall was in the track entry #3 on #2 section - nearest spad no. 21045. Fall was 20'L x 20'W x 54"" thick approx. 4,000' from the face. A clean-up plan is posted and fall was reported to both State & Federal Emergency Hotlines. No injuries occurred. No one was trapped.

A roof fall has occurred in the Second Southeast Mains in No. 3 entry at Spad 23+10. The dimensions of the roof fall are 50' long, 18' wide and 5' high.

The east finley elevator was down for more than 30 min due to a door interlock failure. There was no entrapment.

A fall of ground occurred in LW59C inby section. The fall of ground was approx 40'long X 30'wide X 2'-10' in depth. The fall of ground occurred some time after 10pm on 12/29/07 and was found mid morning on 12/30/07. It was inspected by safety engineer and mine manager. Was determined reportable at 11:45am.

A second unplanned ignition of coal dust occurred in the #2 coal mill circuit while trying to re-start the coal mill. There were no personnel injuires or property damages.

On 12/29/2007 an unintentional fall of roof occured on the 4th North Sub Main in #5 entry between cross cut 88 to 89. The fall measured approximately 16' wide x 35' long x 6' to 8' thick. The area was bolted with 6' point anchor roof bolts. The fall will be cleaned up as approved by MSHA.

An unplanned ignition of coal dust occurred in the #2 kiln coal mill circuit causing the two explosion vent panels to open and vent pressure. No personnel injuires or property damages.

The top is falling out 9? to 12? out in front of the tips shields #1 thru #5. Top falling high, started out at or about the horizon of the anchor point of the original bolts. #1 shield tipped over 1/3 to ? from being on its side. We installed ?I? beams, cable bolts, truss bolts, square sets, rok-props. However the fall kept flushing higher and higher.

A roof fall was discovered on the 5th Southwest, Entry #2, Sta. 11+50. The fall measured 70' L X 19.6' W X 8' Thick. The area was developed on 5/8/07, and bolted with 8' point anchors. The fall did not affect ventilation or passage of men, and there was no personal injury or property damage. The fall will be cleaned up.

A methane ignition occurred on the #1 section, in the #4 face, while the miner was cutting on the left side of the face.

A roof fall was discovered during an examination at the mine. The fall was located in the return aircourses on the 3 right panel. It measured approximately 30' in length, 20' wide and 4' thick. The area had been previously bolted with 42"" fully grouted bolts. There were no injuries, no damage to equipment, and ventilation was not affected.

An unintentional roof fall occurred in the tailgate entry of 9I @ 18 x-cut. The Fall measured approx. 30ft long X 15.5ft wide. The fall fell out above the anchor point. ****No injury or equipment damage***

Cement Kiln was in an upset condition, kiln discharged hot clinker which caught 454 belt on fire. Supervisor discovered fire at 7:02 am and called the fire department. Fire was extinguished at 7:41 am. There were no injuries as a result of this accident.

Unintentional roof fall occurred in intersection of #3 entry at spad #3831 1700' outby 005 MMU 3rd SW Mains. Fall was 18' wide x 18' long x 8' high, bolted with 6' tension rebar & 8' point anchor bolts; area is cribbed off.

The Crane/Hoist went down with 3-miners in shaft. During diagnostic check it was noted that a fuse had blown. Two After Market Horns had been connected to this circuit prior to the Crane arriving at site, overloading the fuse. The two After Market horns were removed, fuse replaced, Crane inspected & bucket was run up & down the shaft & was determined to be in good condition.

A roof fall was found in 1-Left Bleeder panel in the #2 entry at spad #2102 when making the weekly examination.

Bringing cage up from 4100 level; hoist stopped. Reset hoist, brought to collar with no issues. Hoist down approx 10 seconds. Hoist brought back into service after several test runs, at approx 9:00 a.m.

When traveling the bleeder entry in 1 Left Panel off No. 11 belt a fall was found in the #1 entry. Fall fell from pillared works into the travelway.

An unintentional roof fall occurred in the 1A neutral entry #8. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 50' x 20' x 7' thick. The fall will not be cleaned up.

A roof fall was discovered on the 3rd MNE, between #2 and #3 entries at Sta. 24+85. The fall measured 20' L X 19' W X 8' Thick. The fall did not affect ventilation or passage of men, no personal injury or property damage. The area was developed on 2/1/2007 and bolted with 8' point anchor bolts. The fall will not be cleaned up.

A rock fall occurred in the 4rg SE submain in #4 entry (belt entry) spad #27+56. This area is outby #2 unit working face approx 1500 ft. Fall size 25' by 75' x 8'. Type of rock - shale.

An unintentional roof fall occurred at the Galatia North Portal Crosscut 11 on the Flannigan Main Travelway, #2 Entry. The fall was approx 18' long, approx 16' wide and approx 10' thick. The fall did not affect ventilation. The fall will be cleaned up.

An unintentional roof fall was discovered at 6:15 AM on 12/26/07. The fall was located at Break #31, #4 Entry on SW Main at Spad #174. The approximate measurements were 20' wide x 25' long x 5' high.

A roof fall was discovered on the Main NW, Entry #4 (beltline)sta. 8+40. The area was bolted with 5' fully grouted and 12' point anchor bolts, developed on 7/1/2002. The fall occurred in a 4 way intersection, it did not affect ventilation or passage of men. We did have property damage to the belt and structure in the area of the fall. It measures 30' L X 18.5' W X 12' + thick.

Unplanned roof fall in 2nd North Butt Room, #12. The fall is 80' long, 20' wide and 5' high. Inby spad #3119. The place was bolted with 42"" fully grouted resin bolts.

One of our exmainers found a roof fall in a worked out area on the 2 Left, 5 West worked out panel. The fall was located at crosscut 37 between entries 4 and 5. The fall was in an area adjacent to the examiners route of travel.

A roof fall was found at x-cut 62, #4 entry, in 4th East. This is an outby area of the mine and is not an escapeway or travelway. The roof fall is not impeding ventilation. It has been dangered off and additional support set on both sides of the fall. The fall is approx. 15' wide x 40' long and 6' high. The area was supported with 5' fully grouted resin bolts.

A roof fall was discovered in the #3 entry of the 9E submains off of 9 South.

Employee was working in the pocket. When he was coming down in the elevator, the elevator stopped between floors. He was in there approximately 40 minutes. He was fine and was not injured.

A fall of the roof occurred in the 2nd SE submains at crosscut 72. The fall measured 50' in length, 19' wide, 6'-8' high. No ventilation, equipment or personell was affected.

EE was operating the cage in inspection mode when it stopped due to a sensor malfunction. He was removed from the cage at 11:10 AM after Schindler Elevator Service corrected the problem

The Kuhntown Elevator went down while an employee was doing his safety checks. Three rectifiers were found to be bad and were replaced. Also a DC connection was loose and tightened. The elevator was back in service 12-22-07 at 1:40 p.m. No injuries occurred.

Blizzard conditions developed in the morning of 12/22/07 at the mine. Operations were running as a typical production day. As the high winds continued from the blizzard, the local power lines were briefly shorting out power to the mine. On one such occurrance, the power remained out. After 15 minutes with no power, MSHA was called and miners brought safely to the surface.

Fuse for a fan used to cool a drive cabinet blew, which did not allow the hoist to re-set. Fuse was identified and replaced. Began evacuation of the mine, but did not complete as the down time was under 20 minutes.

A roof fall occurred in the #9 seam in the 7 East Panel at crosscut #14 in the intersection of #7 entry extending inby and outby for 25 feet measuring approximately 60ft. x 18ft. x 9ft. high. This area was supported with 5 foot fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

A roof fall was discovered on this date. The fall is located in 8-Right, Break #9, in 2-Left crosscut. The fall measured 18' X 20' and was 6' in thickness. The roof was previously supported with 5' Hy-Tech bolts. The fall will be cleaned up and supported with super bolts. Ventilation was no affected and there were no injuries.

Due to a power surge the MOV (metal oxide varister) of the Avtron electronic drive burned out shutting down the #4 Mine Hoist. The mine was safely evacuated via the #2 hoist (second way out). Night shift has been canceled.

1045 fall occurred 001 section #6 entry spad #20693. Entries to fall have been dangered off; fall will not be rehabilitated. Fall was 70' x 6' x 7'; both MSHA and OMSL were notified.

A roof fall has occurred in the second Southeast Mains between No. 4 and No. 5 entries, at crosscut 32. The dimensions of the roof fall are 35' long, 18' wide and 10' high.

An unintentional roof fall occurred at the Galatia North Portal Crosscut 29, 4360' 1West Longwall #1, #2 and #3 Entries. The fall was approx 20' long, approx 300' wide and approx 10' thick. The fall did not affect ventilation or the passage of men. The fall will be cleaned up.

Escape Hoist went down with a Hydrolic Pump in Zone 4. We moved the mine crews over to Zone 2 where the Escape Hoist is working, until we get our hoist running in Zone 4.

A roof fall occurred on the travelway of the 1st west panel, entry #6 at 41 x-cut. The fall did not affect ventilation, travel or cause injury. The fall was 25 ft long, 18 ft wide, & 10 ft high.

Seal in the middle of a brake drum caliper broke leading to a hydraulic leak. There are a total of 8 calipers, so braking was not affected. Since this occurred on the #3 Man Hoist, the mine was evacuated. Total downtime was about 5.5 hours.

A roof fall occurred in #7 head drive, 25 feet inby spad 1723. The fall was 4.5 feet thick, 18 feet wide and 30 feet long.

While pushing overburden in the east end of the west pit, a bulldozer hit and detonated a primer. The only damage sustained was a broken lense on left corner blade light.

A roof fall occurred in the 1 west 1 mains, 39 crosscut, #7 entry. The approximate dimensions are 8' long, 8' wide and 3' thick. The area is supported with 5' fully grouted resin bolts and 6 X 6 timbers.

A roof fall was discovered in the 2 north 3 mains 7 crosscut #2 entry. The fall measures approx 20' long, 18' wide and 5' thick. The area is supported with 5' fully grouted resin bolts and 10' paint anchor bolts.

A roof fall was discovered on 12/18/2007 at 2:00 am. The fall was located on the 5th Southwest at S.S. 18+30 entry #6. The fall measured 25'W X 25'L X 9' thick. The area was developed on 5/7/2007 and was supported with 8' point anchor bolts. The fall DID NOT affect ventilation or passage of men, no personal injury or equipment damage occurred. This fall will not be cleaned up.

An unintentional roof fall was discovered by foreman in old works. Fall measured 20' wide x 70' long x 8' high. Fall located between 40 cross cuts out by #2 section near spad #4605.

A fall of the roof occurred on the 1 D belt line, 3rd S.E. panel. The fall measured 20 ft L x 50 ft W x 7 ft thickness. No ventilation, equipment or personnel was affected.

At the shift change during the pre-shift inspection the hoist operator noted that while he was lowering the bucket on the surface it appeared that the brakes were intermittently stopping the lowering process. He called for the mechanic and the equipment superintendent, who was not immediately available. The 5-man crew in the shaft was told to stop work and evacuate.

Fall was discovered in the No 2 entry of 3 west marker at survey station No 5395. The fall measured approx 18' X 20' X 5' and will be cleaned up. Ventilation and passage of men was not affected. There were no injuries.

An unintentional roof fall was discovered by foreman in old works. Fall measured 19' wide x 100' long x 4' high. Fall located at cross cut #21 in #3 entry next to the #3 belt entry. Did not fall on belt conveyor.

At 4:08 PM an interruption in the electrical supplyoccurred, which we believe was a surge in power and to which the power company said likely did happen. Result was damage to a circuit board panel in the DC converter of the hoist system. A new circuit board was flown in on 12/18/2007. Following hoist checks and inspections, the hoist was put back in service at 7:45 PM.

A roof fall occurred in the D-Mains section of the mine in a x-cut between the intake & belt entry. Approx 30' long x 6' high. Area was resupported. Location is 1 x-cut inby station #3701.

The slope car servicing the #1 track slope was inoperable from 12:32 p.m. until 1:28 p.m. due to a malfunction in the contactor.

An unplanned roof fall occurred in the #130 mine on the old # 9 belt in the #5 entry 2 brks. inby spad #7862. The fall measured 45' long, 4.5' thick and 19' wide. The area was developed on 10/29/07. No injuries were reported.

Miner operator was trimming bottom when he noticed small blue flames along a portion of the floor. Extinguished flames with water. NOTE: This report was filed electronically on Sunday 12/16/07 from an airport location while traveling, but in checking the records today there is no evidence it was ever received by MSHA, so it is therefore being refiled.

A roof fall was discovered on this date. The fall was located in the #3 entry, #4 crosscut of 6-North. The fall measured approximately 20' x 30' x 12' in thickness. The fall did not affect production or ventilation. The area was previously supported with 5' hi-tech bolts and 8' super bolts. The area was cribbed and dangered off.

Alt Solid Fuel fire caused by heat buildup in 2nd shredder. Heat was caused by buildup of low density plastic in shredder screen which caused material to be over shredded. Metal was also present in 2nd shredder screen which may have provided a spark. Any of these conditions may have lead to the ASF catching fire.

A section of the south wall, approximately 25'L x 8'H x 4' to 6' deep in the 5900 16 East stope fell in against the stope jumbo. The existing back (cemented fill) was intact. The operator on the jumbo was not injured. Additional ground support had been installed in the area.

The powdermen were loading 22 Right Cross Cut in 2 Yard South when the compressor running the ANFO Pot caught on fire. The fire engulfed the truck, compressor, and ANFO Trailer. The mine was evacuated. The exhaust was monitored until the air reached safe limits and then the mine rescue team went into the mine and examined the fire scene to determine that the area was safe.

An unintentional roof fall occurred in the 1A return at station 49+07 between entries 1 and 2. The fall did not affect ventilation, affect passage of men or cause personal injury. The fall measured 65'x18'x6-8' high. The fall will be cleaned up.

As water receded in the old Northwest Mains area during pumping efforts a roof fall was identified in the #1 intersection that extended through the crosscut to the #4 intersection inby spad #4163,4162,4161,& 4160. The fall was approx. 20'L X 250'W X 5' H. The area contained 4' & 6' Dbl. lock bolts and was mined approx.1/94. The area has been dangered off and timbers have been set.

A loss of power occurred to production hoist. One side would power up; other would not. MINE WAS EVACUATED. After much troubleshooting, a faulty contact was found at the Joystick controller area. Part replaced; operation checked. No injuries or other problems. Back in service at 9:30am.

Rock fall occurred in 1 East Mains at break #17 in the crosscut between the #4 and #5 entry. Fall measured approx. 20' wide x 25'-30' long x 5'-6' thick.

We had a Long Bolt Crew attempting to bolt, when we had a ground fall occur pulling past the anchor point of our exsisting split bolts and wire. This was an immediate report to MSHA, which was done and the area was signed off.

No injuries - garage and shop fire. Waste oil burner is the cause according to the fire marshall. Insurance company will take burner and inpspect it. Total damage to the garage structure 24'x 54' and 24' x 65'. All equipment and supplies are lost plus lots of tools. Shop has minor damage mostly to the roof.

When the operator, using a torch, attempted to burn an opening to install an idler on the conveyor belt, the flame apparently ignited soot build-up under the conveyor's rain hood from an oil burner that was used to heat the feed hopper. The flame then spread to a second conveyor. It appears both conveyors are destroyed as a result of the fire. Fire extinguished in 30-45 minutes.

Fall will not be cleaned at this time. It is 30' long x 20' wide x 4' thick. it is at x-cut 9 in the main intake. The escapeway is clear for travel and the fall has be checked by MSHA and OMSL personel

The longwall face was mining through #15 crosscut. Between night shift and day shift the crosscut fell in.

A roof fall was discovered in the #5 entry of the R Butt section. It did not impede the normal route of travel to and from the section. Timber was set in various locations in the #5 entry.

Loader operator was mucking out a heading that was shot the previous shift. He scooped up a bucket of material, raised his bucket approx. 3 feet and started backing up when (apparently) an unexploded stick of dynamite detonated. All miners were evacuated from the mine. There were no injuries or property damage. MSHA was immediately notified.

As the continuous miner was completing a pushout, a piece of roof rock fell near the pushout and fouled the machine's cutter head. This prevented the continuous miner from being readily removed from the area. Sometime later, as the machine remained in this position, additional roof material fell onto the continuous miner. There were no injuries.

An unplanned roof fall occurred in the #130 mine at brk. 14 on 7 belt, spad #6292. The fall measured 25' long, 19' wide and 8' in height. The area was developed on 11/17/07. No injuries were reported.

Rock fall occurred in the #1 entry in the first left panel off of 2nd right off 002 sections sub-mains at break #22 through #24. Fall measured approx. 20' wide x 60' long x 4'-5' thick.

Downed power lines, etc. brought about by a heavy ice storm resulted in electricity going out at approximately 11:30 AM, December 11, 2007. The power company finally restored power to the mine location at approximately 5:30 PM, December 12, 2007. Following hoist checks and inspections, hoist was placed back in service at 6:45 PM, December 12, 2007.

An unintentional roof fall occurred on the East pillar section, in No. 1 entry, approx 30' inby survey station 11783. The fall extened from the second lift in the left hand barrier into the entry and fell on the No.3 and No.4 MRS's. The fall was approx 5' high, 10' wide and 20' long.

A roof fall occurred in the 6th east, #42 crosscut, #11 entry. The fall was 18' long, 18' wide & 8' high. The area was bolted with 8' tension rebar bolts. The fall didn't affect ventilation, travel, damage equipment or cause any injury. No one was required to work or travel in this entry.

Employee entered the Moly Plant elevator on the 1st floor. A contractor was washing down the floors and accidentally hosed water in the shaft causing the elevator control fuse to short circuit. Employee was trapped in the elevator between the 1st and 2nd floors for approximately 1 hour while the fuse was replaced.

A roof fall occurred in the old 2 west 3 headings, crosscut 18, #2 entry. The fall measures approx. 25 feet long, 18 feet wide, and 10 feet thick. The area is supported with 5' foot fully grouted resin bolts, and 10 foot point anchor bolts.

The main decline encountered a rib failure between muck bay #3 & muck bay #4. The affected area was along the left rib of main decline. It is approximately 20' in length. The approximate elevation is 5362.

During welding, cutting & burning on the shear bit lugs, methane was ignited and immediate action put out the ignition. There were no injuries reported. Proper practices being followed.

Roof fall on 6c beltline at bk 22-23. Roof fall is apprx 70' long x 20 ' wide x 5' high. The roof fall will be re-bolted and the beltline will be ran over top of the rockfall when rebolting is completed in the area.

A roof fall occurred in the #11 seam in the 2 Main North cross-cut #172 in the intersection of #2 entry and extending through the intersection of #3 entry, measuring approximately 80ft. x 18ft. x 4ft. to 5ft. high. This area was supported with 30 inch fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

An unintentional roof fall occurred 2500' outby the active working section in the No. 3 netrual entry. The fall measured 30' long, 20' wide and 6' thick. The fall does not affect the fravel of men or the air flow.

An unintentional roof fall occurred in the 3C intake at station 41+70 entry #4. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured app. 20'x20'x6' high. It will not be cleaned up. The fall did fall on the high voltage cable in this area.

8O Section #8Break #2 Entry had been mined 05/04/2004 7,663 Feet from 8Q Face. Fall approximately 18' wide, 40' long and 10' high.

A metal contact near a coil came loose (bolt came out and 1 broke off) and made contact with a coil. The power diverting damaged a rectifier, causing the hoist to be inoperable. The mine was evacuated; no problems. The damaged parts were replaced and hoist put back in service at 7:30am.

Roof fall (200-ft long x 19-ft wide x unknown high) found by mine examiner in worked out area R2 off 3 Right. No noticable defects when area mined initially. Pressure cutter now in area.

Belt 612 feeds belt 614 through a steel chute. Fire was confined to the inside of the chute but burned both belts in two. Origin was spon-com of coal on teflon dribble chute at head of 612 which then dropped hot material onto 614. Fire was extinguished within 45 minutes.

An ice storm disrupted power to mine site at 1:10pm on 12/10/07. Power to fan and both hoists were lost. All personal underground traveled to the Elkhart portal and power from the back up diesel generator was used to bring all out from underground. Incoming power was restored at 5:00pm and the mine was reexamined and work resumed.

Tailings pond #823 failed along the eastern wall from decant water piping through a deep linear crack. All water flowed into pond #827. Pond had just been repaired from previous failure.

Operator was dumping load of backfill at the face. While leaving the heading the operator observed a ~5?x4?x8? slab had fallen from the rib. The operator immediately secured the equipment and evacuated and secured the area (roped it off). There were no other operators in the area.

Miners were rehab bolting in P14/ vent crosscut. They were cleaning up to do additional bolting when they noticed the ground starting to move. They backed out and ""flagged off"" the entry. Shortly after, the roof caved in above the anchor zone. Ventilation was not interrupted. MSHA was on sight and issued no citations.

It was observed that the Koepe mancage was about 1 1/2' below the bottom landing. The bottom gate switch was bad. The fill-in surface electrician was unsure of being able to operate the Koepe on Manual. Mine was evacuated; no problems. The midnight shift entered the mine at their normal time via Koepe on manual. The switch was replaced.

The cause was presence of water bearing sandstone.

A possible ignition of methane gas may have occurred in the No. 3 face of the MMU-001 on the North Mains section. According to witnesses, a short orange flash occurred near the center of the cutter head while the continuous miner operator was loading cleanup coal on the left side of the working place. An employee was conducting a daily inspection of the slope hoist & found the hoist was inoperative. A co-worker happened by at about the same time. This was on a Sunday with nobody working underground. Testing showed a field exciter to be open. The motor was down & had to be taken off & sent to a shop for repairs. The motor was installed on 12-3-07 & hoist was working properly.

At approximately 10:00 a.m. on 12-01-07 the hoist motor went down and electricians were called. At approximately 10:30 a.m. MSHA was notified that hoist was down and a man was in the skip station and had a ladderway to escape if needed. Hoist was repaired and returned to service approximately 5:50 p.m. same day and MSHA was notified that hoist was back in service.

A truck got stuck. The loader backed up to the truck to pull it out. The loader operator and the truck operator both got out to hook the chain up. The loader rolled back and crushed the truck operator between the truck and loader. THIS PERSON IS NOT AN EMPLOYEE OF OURS, WE DO NOT HAVE HIS INFORMATION. The loader impacted the loader operator in the back, he moved out of the way.

Fall from machine

Employee and another member were changing a bottom roller on the Cross Hollow belt at the # 130 mine. Injured was laying on the bottom belt changing the roller. The belt suddenly started and traveled approximately 49' before employee fell approximately 39' to the ground. EE passed away on 12/14/07 at 11:30pm.

Employee went to assist in replacing rear cover plate on dozer; plate on floor jack on edge. While jacking up, he lost control of plate and his left thumb was pinched between plate and a solid object.

Employee was turning the bull gears on a belt drive while other man were threading a pull rope to thread the belt thru the drive rollers. His hand slipped causing his right index finger to get caught between the teeth of the bull gears.

While scooping the feeder, the rails on the operators side of the batteries slide forward into the cage around the operator. When this occurred, the operator had his left hand holding onto the cage, which exposed the fingers to the outside of the cage. As the rails struck the cage, the end of one amputated the tip of his left middle finger

In the process of plowing and cindering the haul road with a 1986 International S1800 dump truck, the injured stopped the truck to check on the remaining cinders. While standing on the tire, his foot slipped and his wedding ring got caught on a screw on the side board, causing the left hand ring finger to be degloved, which resulted in the amputation of his left ring finger.

As employee was installing a bolt into a hinge pin collar on the #3 MBM, he was told that the #1 & #2 MBM was going to be moved to pull back Lo-Lo belt. He replied that he would be out of the way and to proceed. When the MBMs were moved, his thumb was caught between the collar and the hinge pin, resulting in the amputation of the tip of his left thumb.

Employee was helping other employees maneuver outby Pinner cable and plug over other cables near the section power center. As he was pulling the plug over another cable he felt a sharp pain in his abdomen. Later diagnosed as a hernia.

The employee was driving Haul Truck #302 a Toro 40D. He had pulled over and stopped to allow another truck to pass coming the opposite direction. The other truck passed and he pulled out crossing to the other side of the roadway and struck the pipelines on the opposite side.

Employee was positioning a guard during a move, backhoe bucket crushed his hand between the bucket & the guard. Two middle fingers on his left hand were amputated and the left index finger sustained a severe crush injury.

EE was posititioned outby the boom of the cont. miner, moving the cable. The cont. miner had completed the left side lift. A piece of rock 4' x 2' x 6"" fell out from between roof bolts. EE saw the rock falling & put his hands up to block it from striking him. It struck his hands, & pushed him backwards. As he fell backward, the rock struck his upper lt leg knocking him down.

Employee was uncoupling rail cars at the packhouse. While uncouping a rail car, the handle was stuck and when he pulled up on the handle, he felt some pain in his left bicep. The original diagnosis on the evening of the 22nd was a strained bicep. A Subsequent diagnosis has determined that he has a deteched tendon in his bicep and will require surgury.

Once employee was able to stand the individual could not stand up straight as they normally would. Cause of this injury was safety awareness, compliance with rules/regs was not a factor, miner proficiency and attitude was a factor, protective equip was not a factor.

The employee stated he was installing a cable bolt at break five when he started to push the bolt into the hole the plate caught on a track tie and caused him to feel pain in his shoulder. The employee did not start missing work until 1/2/08. He was diagnosed with arthritis and severe degenerative changes in the shoulder.

Employee was walking belts; he stepped down into an underpass, twisting his left knee. This was on December 20th of 2007. He continued to work and is working at the present time. The employee went for a MRI and had a positive result. He is working awaiting doctor's instructions.

While in the elevated basket of an Elliot truck, two miners were hand scaling, while mining one piece of rock another piece broke loose and came down the bar and struck miner on the left arm, causing a serious laceration to the left wrist and forearm. Forearm required surgical repair.

He was climbing up in #4 face & moved miner over to the right side. He could not get the ripper head to start. He trammed the miner back out of the face. When the miner came toward him he moved forward to keep from being hit and was pinned between the miner and the rib. It hurt his left arm and torso.

Injured employee was in the process of placing his body inside the battery powered scoop compartment. He placed his hands on top of the scoop canopy and was pulling his body inside the scoop deck when he experienced pain in his right shoulder.

This resulted in EE losing time from work.

A truck got stuck. The loader backed up to the truck to pull it out. The loader operator and the truck operator both got out to hook the chain up. The loader rolled back and crushed the truck operator between the truck and loader. THIS PERSON IS NOT AN EMPLOYEE OF OURS, WE DO NOT HAVE HIS INFORMATION. The loader impacted the loader operator in the back, he moved out of the way.

Employee was going up push arm steps to enter the operator's compartment. As he was doing so his right foot slipped on step thus permitting his body to shift allowing his head to strike on the push arm.

Employee bent down to pick up 100 lb bag of sand. He didn't bend at the knees. When he lifted the bag he felt a pull in his lower back.

Worker was sitting on beam 14 feet above ground helping another worker torque down 3/4"" bolts on dump hoppers. He had torque gun with him on the beam so the other worker in scissor lift could reposition the lift. Once he had the lift repositioned he had the worker hand back the gun and that is when he fell from the beam.

A 988 loader was backing off of a feeder ramp when it struck another 988 loader which was waiting for other mobile equipment in the area to clear. The impact caused the operator of the stationary loader to strike his forehead on part of the rollover protection which in turn resulted in a laceration requiring stitches.

While tightening a bolt on a 8"" vic clamp on a paste pipe line, he was pulling on the wrench when he felt a pop in his right shoulder and it gave out and had alot of pain.

EE was flagging the crane operator while the forklift was being used to pull the crane into gear. The crane went into gear. A 3"" nylon strap was hooked up to the crane and a 3/8"" choker was hooked up to the forklift. The strap and choker were connected with a 7/8"" shackle. Due to too much tension the choker broke. A slingshot effect occurred and the shackle hit employee's leg.

The injured was working on the cats of the #1 Continuous Miner when a piece of roof rock measuring 4'6"" X 4'8"" X 1-6"" thick fell out of a small slip. The rock struck the injured's left leg causing two fractures to his fibula. The injured required surgery to repair.

Resulted in a hairline fracture to his right wrist. Occurred at survey station 300+50 on the mainline. No roof bolts were dislodged.

After washing drill, driller had trouble with his AC unit. After finishing the hole he was drilling, he climbed up to check on the problem. On his way down with his left foot on the track of the drill, his right pants leg snagged on a piece of metal causing him to lose his balance and fall to the ground. Employee was working on shuttle car cable when he went he went to power center to hook cathead back up & try breaker. When he put breaker in, cathead & receptacle started firing up. He received the burns while the arcing was taking place. He stated he pulled the cathead from the box while this was going on.

Later, 1/3/08, the employee reported lingering discomfort in the knee; doctor evaluated and prescribed Celebrex and it became reportable at that time.

Employee strained the lower back using a 60 lb bar to strike the door to #3 ball mill to break the seal so the door could be opened.

Employee was spotting bolts in a loose roof area, along the rib. The drill steel helped dislodge a piece of rock that slid onto the driller's canopy and then off it and onto his left ankle. Rock was 27"" long x 14"" wide and 0-2"" thick (employee worked restricted duty. Dr visit 1/4/08 another MRI showed fracture. Employee off).

Employee was bending down to pick up plastic pipe. Employee stated that he was not doing anything out of his routine duties. Nothing strenuous or unsafe. He said he felt the pain when he bent over.

Employee was working out of a bucket truck swinging a sledge hammer. When he swung the hammer to strike a pin he accidently pinched his left index finger between the handle of the sledge hammer and the lip of the bucket truck at the same time he struck the pin with his bottom hand holding the handle to the sledge hammer. He fractured his left ring finger.

Employee slipped and fell on the deck of a dredge, bruising the right side of his lower back.

Employee was tramming a bolter when a piece of corner rock struck his head causing him to fall into the pin boards, hitting his face and causing a laceration to his arm.

Employee states he was holding up a roof mat to be installed. As he stepped up on the bolter deck, he lost his balance and fell against the roof bolter. The impact resulted in a dislocated right shoulder.

Employee claimed he was reaching for supplies when his knee slipped and slid down between tire and frame of the roof bolter he was operating resulting in his knee being twisted. The roof bolter was stationary at the time of injury.

EE slipped and fell on the ice straining his low back. No treatment was sought. The injury was aggravated five days later and medical treatment was sought. EE was examined by the authorized provider. Meds and PT were prescribed. EE was instructed to stay home from work.

A belly pan was being installed after work was completed on a hydraualic line. The pan was being lifted in place by a cable. Once the pan had been lifted, the mechanic moved into place to install the bolts that would permanently hold the pan in place. As this was done, the cable failed and the mechanic was struck by the pan.

Rock got caught at opening of jaw crusher. EE was using a pry bar to turn the rock. The bar kicked back and pinched his finger between the bar and the guard around the jaw.

The individual was walking down the mine entry when he stepped on a piece of loose coal and twisted his left knee.

Employee fell from truck while exiting and hurt her L-foot. She reported this immediately and finished her rotation. On her days off she became ill with pneumonia went to doc on 13th had foot checked, turned out a small break. I was contacted on the 13th and she was released from doc with illness but had 3 days of restrictions left for foot and could not work.

Truck driver got off her truck at the end of the shitf and realized she had left her bag in the cab, and remounted the truck. As she dismounted she slipped from the ladder, then noticed pain and swelling in her knee. She saw her physician later that day, prescribed a knee brace, referred to a specialist and was given light duty. Currently Ongoing

Primary operator was greasing Jaw crusher. Mechanic was working on rock breaker. He did not realize that the breaker was under hydraulic pressure or that he was below greasing. He was checking a leak on a hydraulic line and when the hydraulic oil leaked out the hammer lowered and pinched operator who was below. No broken bones or internal injuries to operator.

EE was at back of roof bolter eating, when a piece of rock dislodged from roof and fell striking him on his left wrist. EE had a sizeable laceration along with possible fracture of wrist. Amendment: EE went on work restrictions due to this injury beginning 4/9/08.

Repairing in the wash plant changing an 8 in pipe when a piece broke and fell striking his lower leg.

After installing the last roll of roof bolts, the EE was filling his glue box up and a piece of rib rolled off and hit his lower legs.

Over-exertion in pulling or pushing objects

On 12/31/07 Employee was pulling and pushing a head mechanism on a drill press across discharge manifold flanges. This was done so that the flange faces would be the same length; he worked at this task for about 3 hrs. Employee reported he did not have any pain or soreness until the next day. On 01/02/08 he came to work and reported that he felt like he needed to see a doctor.

Employee was removing empty pallet from stack to place in scissor lift. Top pallet was caught on another pallet and employee jerked pallet to get it loose. In doing that as the pallet became free his left hand was caught between both pallets.
