Accident type, without injuries

At 9:02 am an employee noticed flames coming from the rental heater. Three employees tried to use fire extinguishers on the flames but it wouldn't extinguish. They called 911 and the fire dept. came around 9:25 am. and extinguished the fire by 9:38 am. The United rental heater was a 2012 Almand MH1000 which was a total loss.

Smoke was discovered in the 5W coal pit on the highwall. The smoke was not able to be extinguished in 30 minutes. No miners or mine equipment were at risk during this event.

An unintentional roof fall occurred in Panel 4, entry 7, crosscut 40 intersection. The roof fall measured 8'T x 19.5'W x 35'L. The roof fall will not be cleaned up and immediate area was re-supported. The roof was supported with 5' resin roof bolts. Date of development was November 2017.

We had an unexpected roof fall above the anchorage point. There were 5' resin bolts installed. We are in non producing status. All entries to the fall have been dangered and timbered off. The lifeline has been re-routed

1:00 pm #29 Mine hoist went down. After evaluation it was determined to be a skip hung in shaft. MSHA was called and evacuation procedures were in process. Evacuation process completed at 4:45 pm. Maintenance was completed to put shaft back into service on 12/30/17.

One employee in Tilden 2 Pellet Plant elevator for 65 minutes. Communications with employee at all times, no injuries. Elevator was stuck approximately 5 inches above landing. Elevator contractor Thyssen Krupp conducting investigation to determine root cause, upper limits and cold weather as probable causes. T2 Elevator is tagged out of service pending investigation and repair.

All power to all surface and underground operations were lost due to a Cleveland Public Power equipment failure at an offsite substation that supplies power to the west side of the city.

During an idle period the Night Shift Security officer observed a spon com fire in the North Field Stockpile. No other surface personnel were on site. Personnel were called in to extinguish the fire and it took them longer than 30 minutes to get to the mine, requiring an immediate notification and report filing.

A Fall of Ground (FOG) occurred in the N72D5/N72D5A Intersection. An estimated Area 25'long by 56' wide and averaged 5.5' thick. Approximately 675 tons of material. The N72D5A was used as a supply storage area and is frequented daily.

A roof fall occurred in the 1st Panel North off of the 1st Main East in Entry #1 at 52+80. The fall measured approximately 40' long x 20' wide x 8' high. This fall was in the examiners weekly examination route.

An unintentional roof fall occurred in the First Southwest Mains entry seven, between crosscuts thirty-four and thirty-five. The roof fall measured 6'T X 19'W X 60'L. The roof fall will not be cleaned up and immediate area was re-supported. The roof was supported initially with 6' resin/tension roof bolts. Date of development was June 2010.

A roof fall has occurred at break number 33 in the 9N Headgate Entry.

On the I-6 retreat mining panel, a roof fall occurred in the #9 cross-cut of the #4 heading. The fall extended for approximately 60'. The area was originally supported with 8' torque-tension bolts spaced on a 4x4 pattern and 2 16' CC cable bolts installed every 8ft. Due to the location of the fall, the area was dangered off and supported, and the row of pillar blocks skipped.

A Production employee entered the tower elevator at ground level. During ascent the elevator stopped between floors 1 and 2. The employee radioed for assistance and attempts to reset proved unsuccessful. A safe rescue was performed via the roof hatch. A call was made to MSHA when it became apparent the rescue would take longer than 30 minutes.

Roof fall in the intersection of the No. 2 entry and adjacent break No. 3 entry at break No. 26 of the combs mains. 20 feet wide x 65 feet long by 6 feet thick

A roof fall was discovered in the intersection of spad #6718 on the old 10 belt panel. The fall was approximately 7 to 8' in height and the entire intersection. Timbers were set and danger signs posted all approaches. No ventilation controls were affected. No employees or equipment were involved. A map of the area was sent to the local MSHA office.

An Unintentional roof fall occurred in the 5L section. The fall occurred in the belt entry at 91+72 it extended to 91+50. The cause of the fall is unknown, there were adverse geological conditions in this area in the form of layer shale. No injury or equipment damage as a result of this occurrence

Employee was on the Dry Ridge elevator conducting a regular inspection when the elevator stopped due to an over travel fault. The elevator was not operational until the fault was reset by outside employees. The elevator was down and the employee was stuck for an extended period of time.

An unintentional roof fall occurred in the First Northwest Submain number eight entry, between crosscuts seventeen and eighteen in the straight. The roof fall measured 18'W X 16'L X 6'T. The roof fall will not be cleaned up and the immediate area was cribbed off and secured. The date of development was October 2017.

On 12/14/2017 a fall occurred at this mine. The fall occurred at 7 cross cut on #3 belt in the right return. This impeded ventilation for the mine.

A reportable fire occurred on mine property at the substation of the Cameron Portal. A transformer which is owned by AEP was the source. The fire was discovered at 12:40 and extinguished at 1:18 am. No injuries occurred and the mine was evacuated due to loss of power to a fan.

An abnormality of the north production hoist rope was identified in a pre shift check of the ropes. We have voluntarily decided to retire this rope. There is no kinking, no broken strands, and no hemorrhaging of the core. All attachments are in 100% ok condition.

At approximately 11:40 a.m. an airway man identified a roof fall in the right return off of 1 section. Both agencies were notified and no one was injured. The stopping line was set over and all approaches were cribbed or timbered off.

Fall was discovered I-sub belt 9 crosscut. Fall was approx. 25ft long, 16ft wide, 42 inches high area was bolted with 8ft tension rebar and 16ft passive cable bolts area took weight and bolts broke causing fall. No ventilation, equipment or person affected.

An unplanned fall of an overcast supporting wall was discovered at 6:50 pm at the No. 14 crosscut of the 5 North No. 1 Belt. This was not a fall of the roof as no roof was involved only the supporting wall of the overcast. The wall was rebuilt and the travelway was reestablished.

An unplanned roof fall was discovered at crosscut 17-18 between the track and belt entries. The area is approximately 10' high, 26' long, and 16' wide. The fall did not affect any travel ways or ventilation. The area has been posted and dangered off.

A WA600 Komatsu wheel loader caught fire. The mine was evacuated and MSHA was notified.

A face ignition occurred while the roof bolter was drilling through a metal strap against the roof.

A main power outage occurred at the mine site due to down power lines. This caused the whole location to be without power. The crew underground was sent to the #1 shaft to be brought out of the mine. This shaft is equipped with a back up diesel generator. The generator did not start. At approximately 2pm the generator was repaired and the crew was brought to surface.

A fall of the roof occurred on the 1st NE Mains (1st54), #4 entry (belt entry), crosscut #11, intersection. The fall measured approximately 20' length by 16' width by 12'-16' height. Additional support has been installed in the area with steel arches through the intersection.

Employee noticed smoke coming from the top of #6 coal silo. Further investigation revealed a smoldering coal fire inside the silo. Site personnel installed a fire hose and began quenching with the plant water truck while the fire department was called as an additional precaution. The fire was under control and the fire department departed at 2:05pm.

While hooking up cars to hoist out of mine the hoist rope was damaged. Repairs were made and the hoist car went back in service on 12/07/2017 at 2:30 am.

Roof fall in the intake escape way inby No.4 belt drive, modified to restore power to No.4 Drive & to support both sides of the fall. Fall 8'-12' thick occurred at survey station 133 #4 intake entry #2 northwest mains. Roof is supported with 72' point anchor pattern bolts, 8' super bolts & 10' able bolts. Fall dangered off work, travel , power & airflow has been redirected.

During regular inspection of the hoist rope, damage to this cable was detected and the decision was made to change the rope out. This made the Crabapple hoist out of service for an extended period of time. No specific reason was found that caused the damage to the rope. (normal wear)

IGNITION - Occurred on the No. 10 section while Roofbolter #144 was drilling in the No. 4 face. The operator was drilling in hard roof and when ee pulled the bit out of the hole, the ignition occurred lasting approx 30 seconds and was extinguished with one fire extinguisher.

# 2 hoist tripped out due to an electrical issue and was down for more than 30 minutes. Someone had to drive from Winnemucca to fix the issue and the hoist came back up at 6:50 p.m.

An unintentional roof fall was discovered during the weekly airway exam. In haste to report the fall, the MSHA hotline was notified prior to the investigation. Upon investigation, it was determined that the roof fall fell under PPL P12-V-03 and was not considered reportable as the fall was in an entry not normally traveled with a common traveled entry in a single air course.

Contact with hot objects or substances

A fatal accident occurred on December 30, 2017. The employee was driving a Dodge pick-up in a stockpile area and pulled in behind a Caterpillar 992G loader. As the loader reversed, it struck the pick-up which then caught fire. The resulting injuries were fatal.

Two bulldozers pushing in the same area, operators talking on CB. D9 operator is injured worker. D-11 is colleague. D9 stopped talking. D11 found that D9 had gone over highwall. Injured worker passed away from injuries sustained.

Over-exertion in pulling or pushing objects

Employee was kneeling down changing a tire on a pickup in the shop, while doing so employee's legs fell asleep. Employee went to pull themselves up by grabbing the side of the pickup and while doing so employee said they felt a tear in the lower abdomen.

Struck by flying object

The employee ran out of hog rings in the clip gun. When the employee sat the gun down, the clip gun went off and severed the tip of employee's L index finger.

EE slipped getting of 725 haul truck catching wedding ring on the no slip grips on steps, tearing finger. EE was transported to hospital but surgery was put off due to doctor being needed in a more urgent matter. On 12/20/17 EE was operated on where they cleaned up wound & removed top digit of left ring finger. All went well & EE on way home at 2:30 pm.

Was changing hammers on Stedman impact crusher. Ratchet slipped and finger was caught between 3/4"" drive ratchet and outside edge of crusher. Resulting in a loss of distal phalange index finger.

Caught in, under or between running or meshing objects

Grinding Operator reached through a gap between two guards to remove a plugged spray bar at the 2B conveyor. While attempting to remove the spray bar, it contacted the conveyor and wrapped around the bend pulley. The spray bar was ripped from the operator's hand resulting in a partial amputation of the operator's right ring finger tip.

Inadvertent movement of load crushing contractors fingers in wire rope rigging resulting in the amputation of pinky finger.

Inj was in the process of pulling dolly slack when the synthetic rope broke and struck inj in the lower right leg resulting in a fracture to the Tibia and Fibula bones

Employee alleges while stepping into haul truck EE felt a pain in EE's lower back.

Miner slipped on Kabota tractor platform while mounting the vehicle. When the miners foot slipped it got caught between the platform and engine.

The employee was walking around their roof bolter when the employee stepped in a hole and twisted and rolled their ankle.

While exiting the loader EE slipped from the ladder, landing on EE's feet on the ground.

Employee was struck by a rock resulting in shoulder/lower back sprain/strain and restricted work days.

Employee was using a hand grinder and got a piece of grit/metal in eye.

Caught in, under or between a moving and a stationary object

Employee was installing six foot fully grouted bolt in #1 entry on #1 Unit 16+23 in the 6th south panel. While installing bolt six footer, Employee was double booming bolt to attempt to install, smashed hand between mine roof and bent roof bolt employee was putting up. Injury to three fingers required stitches.

Employee was walking across parking lot and felt pop in knee. This was deemed NOT a workers comp case but then review by insurance providers and determined by laws in the state of New Mexico it was a working comp case. This is the reason for the late reporting.

Employee walking in muddy area slipped and fell, hurting left shoulder. Given meds and restricted duty.

While performing work on the Pug mill conveyor and tail pulley, an employee was utilizing an air lance to remove material build up. Ice dislodged from the wand causing the employee to lose control of the wand. At some point the employee injured the left wrist which resulted in multiple fractures.

Employee was checking hydraulic fluid in sight glass. Employee had 3 point of contact moving towards tank when feet slipped on wet track, EE fell and landed on EE's right side resulting in dislocation to EE's left shoulder.

An operator was weighing a railcar, while an employee was spotting the railcar by track 6 and 8 during the early morning. The employee stepped an uneven ground, and twisted knee.

Fall down stairs

Loading customer truck. The air line to the gate valve froze due to subzero temps. EE attempted to access platform, carrying a weed burner to thaw air lines. EE's hand slipped off the rung, EE fell backwards, left arm struck handrail. EE then fell down the set of steps.

Employee had pinned the belt together for the belt move and crawled out from under the belt structure. As employee was straightening up to stand employee felt pain in lower back and left leg down to knee.

Employee was cleaning muck and coal fines at the west main belt discharge area when ee bumped head on a pipe causing ee to lose balance hitting right forearm on belt structure.

EE was removing a rock from a haul truck in the shovel pit area when EE slipped on some rocks and felt a pop in knee. EE was given restrictions.

Over-exertion in lifting objects

A fully trained miner strained lower when carrying rollers from a flat car. The injury was not the result of any compliance issues nor were any protective guards involved.

EE stuck hand in between chopper blade and rock as chopper blade came down and it smashed it. EE said EE knew better that EE didn't know why EE did it.

Employee was walking out to start loader for the day when employee slipped and fell on left arm.

Employee had turned off the feeder, pulled rock up on the side of feeder, then turned the feeder back on. Employee then reached up to pull the rock over to bust it up and a rock flipped up and hit a finger.

Was going down steps when employee felt pop in right knee. Employee did not start missing time from this injury until January 15, 2018.

A mechanic injured a knee dismounting a pickup truck. Employee sought medical attention on 1/19/18 and was not released to return to work.

On 12/14/17 EE stated EE felt a pop in EE's left knee & did not recall a specific event at the time. Seen by site medic, referred to ER, X-rays negative, full work release issued. 1/3/18 seen Orthopedic doctor, MRI done, tear was found in meniscus tendon, full work release issued, surgery set for 2/13/18. Employee stated this was chronic not a event & first shift missed 2/14/18

Over-exertion in lifting objects

Employee was unloading belt roller from supply car to front end loader in the Benwood supply yard when employee felt pain in right elbow. The employee did not miss work until 2-6-2018. Cause of injury was overexertion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Fall from scaffolds, walkways, platforms

The employee carrying out the replacement and mounting of the bearing at the central screw conveyor of the Bag house, climbed to a platform with approx. height 3'7"". When employee tries to get off it, slips and falls to the ground hitting back and shoulder blade on the right side.

Employee was reaching up for the stop/start switch for the airlock doors on the track and felt a pain in EE's arm. Started missing work on 2/14/2018.

A fully trained miner stepped on a bent rib strap causing it to strike EE in the right knee. The injury was not the result of any compliance issues nor were any protective guards involved.

Over-exertion in pulling or pushing objects

Employee was pushing open the air lock door when the employee heard something in shoulder pop resulting in sprain/strain injury and lost work days.

The employee was walking when EE tripped and fell. EE claimed to have felt discomfort in shoulder from the accident. The employee did not begin to lose time until 12/29/17. The following were not a factor: Rules, Regs, Skills, Proficiency, Training, Equipment, Protective Devices and Clothing. Attitude may have been a factor.

Struck by falling object

A fully trained miner was walking under the head gate shields when a piece of rock fell from between the shields striking right foot. The injury was not the result of any compliance issues nor were any protective guards involved.

Miner task was to remove and replace steel Grate Decking (1' x39""). EE set down the old decking and foot slipped into the opening. The miner was wearing fall protection.

Plant operator was helping with a screen change. EE was tightening bolts with right hand and had EE's left arm resting on screen. Being that EE had previously dislocated EE's shoulder in April of 2016, it was weak and again slipped out of socket. this is a reoccurring injury that did not result in lost time.

Employee stated that employee lifted tubing to raise and level when employee felt a sharp pain in neck on the right side down through shoulder and midway down side.

On 12/20/17 an employee out on overtime was carrying a hydraulic hoses while walking on TBM catwalk, when they slipped & fell contacting the left knee on the grating. On 12/27/17 the company was informed the employee was seen by a physician & it was determined they had a hairline fracture to the left tibia where it connects to knee, restrictions were issued.

A fully trained miner was carrying belt structure when hand was caught between the piece of structure and a steel set on the belt line. The injury was not the result of any compliance issues nor were any protective guards involved.

Struck by flying object

Employee was positioned approx. 20' to 30' behind the tail of the miner. The coal chain jammed on the miner and when it let loose, something came out striking employee in the left knee.

Exiting rock truck, caught back of boot on 2nd step, lost balance & fell forward hitting head on fire ext. bracket.

Over-exertion in lifting objects

Employee was picking up a bucket of belt chains, as ee lifted ee strained right side of body, rib area. EE began missing time on 1-9-2018.

Struck against a moving object

Haul truck and dozer operator lost sight of each other and made slight contact (no damaged to equipment reported.) ERT members conducted precautionary exam on operators, and dozer operator's vitals were elevated. Employee was sent to the ER and diagnosed with cervical strain, taken off work one shift, then started long days off.

The employee went to personal doctor for a pain in the right elbow and the doctor told the employee to report to employee's supervisor.

A fully trained miner was installing a rib pin when after completing the installation EE turned around and struck EE's hand on a drill steel that was laying on the miner. The injury was not the result of any compliance issues nor were any protective guards involved.

Fall from machine

Employee tripped when exiting the golf cart the employee was riding in. Employee landed face first on the ground. This causing a constant, sharp pain in lower back and bruising on lower lip.

While cutting angle iron with a 4"" grinder, employee lacerated right index finger and required eight stitches.

Employee was riding in a manbus to the section, when a piece of rock fell hitting employee on left shoulder and left knee causing contusion on both.

Employee was stepping over a large clevis & felt & heard a popping sound in left leg. Employee has a strain to employee's left calf muscle.

Employee strained EE's left knee while installing resin in a roof bolt hole. EE continued working until EE had surgery on 2-22-2018.

Employee was stepping out of a Lo Trac and stepped down on a small piece of wood causing employee to fall injuring right knee.

On 12/16/17 employee operating jackleg machine after drilling a hole ee released leg pressure & as machine was coming out of hole it started to spin catching left little finger between throttle & muffler of machine. Seen at the ER, X-rays were negative. On 12/26/2017 was seen again by physician diagnosed with mallet finger, restrictions were issued.

While traveling on haulage, pole came off of trolley wire, then came down striking the EE on hard hat. Did not involve any compliance issues. Personal protective equipment was being used. This did not involve job proficiency or attitude. Employee called off sick on next work day. Did not know time in question was injury related until 12/26/2017 when EE brought doctors slip.

Filling pod duster on the Long Wall monorail, was checking gasket when lid shut on employee's right small finger. I made a mistake, I didn't push the submit button for this accident, I have the copy in my folder.

Struck against stationary object

The dozer operator got off dozer to talk to a truck driver. The operator stepped on a lump of kaolin, rolled left ankle. The operator had a fracture of the left ankle.

Employee was pulling coupler cut-off on the general mine car when the cable broke, causing employee to lose balance and fall injuring right shoulder.

IE was replacing a roof bolter hyd fitting near the base of the ATRS. IE completed the work, the control side operator approached, did not see the IE and thought maint work was completed. The operator started the machine and swung the boom in pinching IE between the boom and ATRS. IE was treated at the local ER for a broken clavicle and released.

Over-exertion in pulling or pushing objects

Employee was installing a roof bolt by operating a walk-through bolter. As ee was pushing the bolt into the drilled hole, ee felt pain in left shoulder. EE was placed on restrictions on 1-9-18 that restricted from performing normal occupation. EE began missing time on 1-9-18.

Employee stated that ee went underground to make the face area. On ee way back outside on the number 5 belt line ee fell hurting back and left arm

EE noticed build up of debris on conveyor #10. EE approached conveyor #10, climbed on bulldozer in order to reach the bolt, removed an bolt from the guard, swung guard out of the way so ee could reach in & knock debris off w/o shoveling conveyor. Arm was caught on pulley, require surgery.

WHILE BACKED IN TO FUEL CAT EXCAVATOR, EMPLOYEE DIDN'T NOTICE BIG DRIED UP CLUMP OF DIRT ON THE GROUND OUTSIDE THE CAB OF SERVICE TRUCK, WHEN EE EXITED THE CAB EE TRIPPED OVER THE CLUMP OF MUD AND FELL ON TO EE FACE AND LEFT SHOULDER.

Employee stated that employee was attempting to lift a shield on the continuous miner when the shield slipped and struck employee on employee's left middle finger causing a fracture to the finger.

Injured was using a sledge hammer to create a hole in a return stopping for a longwall air change. The employee was attempting to control the sledge hammer with non-dominant hand and lost control, which caused employee to hit finger against a mandoor, resulting in sutures to left index finger.

Over-exertion in lifting objects

The contract employee was hanging miner cable during a power move and felt a pain in shoulder. EE was diagnosed with a strain and was not able to fulfill normal work activities.

Individual was inserting a drill steel in the drill guide when aligning the steel, they caught the left index finger between the steel and guide. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve attitude, job proficiency or machinery.

Employee was operating #5 stinger ride on #1 unit traveling outby the last open in the #5 entry. The employee drove thru a line curtain and was struck by a rock. The rock was approximately 4'W X 5'L ranging from 1/2"" to 5 1/2"" thick. No indicators of adverse roof and roof support was installed IAW approved roof control plan. The rock fell out between middle roof bolts.

Injured was pulling supplies with a Brookeville diesel locomotive. Injured stood up, looked inby to check trip, struck the back of head against a steel-set beam and then struck lip against the locomotive windshield. This resulted in a laceration to lip, requiring 20 sutures to close.

Three miners were cleaning up. One was operating a wheel loader and the other two were throwing scrap steel into the bucket to be taken to a dumpster. The loader operator thought one of the other miners signaled operator to lower the bucket. Operator then lowered the bucket onto one of the miners foot.

Employee was loosening the ball valve on the Deister Triple Deck Wash Plant. Employee was using a pipe wrench and felt a pain in this shoulder. Employee left work and consulted a Doctor.

Injured was cleaning the belt entry, EE picked up a large rock and attempted to throw it over the main waterline onto the belt. As EE threw the rock, EE felt a crack and/or a pop in shoulder and elbow.

First responders were called and determined that the employee sustained a non-life-threatening injury to the leg. The employee injured knee as EE fell after being struck by the end of a belting roll. Employee was un-banding the roll preparing to un-wind the roll and didn't realize the roll end was on top. The free end unexpectedly slipped down when un-banded and struck our employee causing EE to fall and injure left knee.

Employee stated was drilling hole in 4L cc. While installing finishing steel in the hole, a piece of rock fell pinching right thumb between the rock and pusher steel causing a crushing injury. EE was transported to Welch hospital. EE was transferred to RGH where EE was admitted overnight. EE underwent surgery and was released the next day. EE remains off work at this time.

Employee was repairing a loading machine tram chain in the 5 West area. When pulling the chain back together employee felt a pain in left elbow. Employee underwent surgery on 12/20/17 to repair a left bicep tendon. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.

Quick connect coupling was loose and went to tighten and blew the coupling. The Employee was struck in the left side of the chest by a bull hose and water under pressure, pushed them back and they fell on the track rail.

Mechanic was assisting the welder in picking up a plate of metal (4'x10'x1/2""). Two plates were stacked on top of each other on the ground. Employee tried to lift top plate so the forks of the bobcat could slide under top plate. When ee attempted to lift this plate, ee heard a popping sound from the lower right side of back. Employee collapsed onto the ground in pain.

Employee was drilling a 3/8"" hole in a beam to give the torch a place to start cutting from and a piece of drill shavings went into left eye.

The rock came through the right-hand door glass and struck the employee on the right hand. The results of the fall were the employee has 2 broken bones and a laceration on the palm of right hand.

Employee was running excavator digging approx. 100' down slope from rocks. Rocks slid onto and into cab of excavator and landed on employee's legs.

A fully trained miner injured their back while moving/pulling a ventilation curtain that was covered in loose coal. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was setting props on belt line, when rib rolled out on employee's leg resulting in a fracture to lower leg.

EE was working off of a ladder on the 18 east section. When dismounting the ladder, slipped and fell backwards, hitting lower back on a bucket of plaster located in the area of the ladder. EE finished shift with soreness/tightness of lower back. EE reported to next scheduled shift but requested to follow up with hospital the following morning.

They were removing a steel pipe at the garage. While moving the pipe it struck the employee on the top of foot causing a bruise.

Employee stated that while hanging cables they felt a sharp pain in the elbow and pain increased as they continued to work. Employee did not violate any regulations, was properly trained and wearing appropriate PPE.

A fully trained miner was struck by a rock when installing glue in a roof bolt hole. The accident was not the result of any compliance issues nor were any protective guards involved. The employee received medical treatment as arm was glued for the laceration ee received.

Accident report not filled out with the mine operator until 12/8/17. Injured was taking an air drill from 64 to 59 1/2 block on walk side of 11 belt when EE tripped, fell down and rolled. EE injured EE's shoulder in the fall, being diagnosed with a pinched brachial nerve in EE's right shoulder area.

Employee was walking outby the continuous miner when employee tripped on two rocks lying on the mine floor causing employee to fall. The employee fracture four bones in right foot.

Employee was trouble-shooting a pump to see if motor was bad or the bearing housing. When they shut off pump when it slowed down and stopped they grabbed the belts and it started to drain the water out of the line which started coasting in reverse and took their thumb through the sheave cutting the thumb.

Employee was reinstalling a 4160 breaker when an arc flash occurred causing burns to the hands, face and arms.

EE was drilling hole into mine roof to install roof bolt, after drilling ee was removing drill steels when the top drill steel fell out of the roof striking ee on the finger resulting in a laceration, cause of accident was lack of concentration. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency was a factor.

EE was working at the 17 East/MN3 header. EE was working with another employee was turning the belt off to remove a rock from the chute. Before the belt was turned off, EE attempted to remove the rock and EE's hand was caught between the rock and the belt. The rock was removed and the hand was released from the belt and rock and EE was brought outside for an ambulance.

IE was roof bolting on unit #2 and had installed two rows in crosscut 2 3. On the third row, after installing roof bolt closest to the rib, the IE started lowering the drill pot down. As the drill pot was being lowered the IE sat the drill pot down on their right foot. IE was brought out and taken to hospital, and was diagnosed with a fractured little toe.

Employee was waiting on a coal Hauler to return to the miner and had backed the miner up in supported top to get a snack and drink from their dinner bucket that was on top of the miner. When employee was stepping away from the miner a section of draw rock fell out striking them in the back and lower left leg.

During morning hours with moderate lighting and overcast skies, employee was on the engine deck checking engine oil level. To better see the oil level, EE turned toward the front, missing step and fell approximately 7 feet to the ground, landing on back. The engine deck was clean and the ground was clear.

12/1/17 EE & another EE was removing a piece of metal from chute when EE started experiencing pain in lower back. EE completed shift and filed an incident report that lower back was hurting, EE worked the following shift. EE sought medical attention on 12/2/17 and given full release. 12/7/17 went to different hospital and taken off work. Pain management for back since 8/2/12.

Employee was walking around a set of saw-horses while cleaning in shop. Employee's foot hit the bottom of a saw-horse leg causing EE to trip and fall to the concrete shop floor. Employee tried to catch self by putting left hand down as EE fell. This caused injury to wrist, hand and index finger. (Broken). Was not reported until next day.

IE stated while squatting to look at a continuous miner a rock measuring 16.5"" X 5"" fell from the roof and struck IE on the hard hat. When this occurred IE lost balance, fell forward, and reached out to brace self. On 12/27/2017 IE was diagnosed with an elbow strain and given work restrictions.

Technician was replacing a step on the pusharm of a D10. Tech tack-welded it in place and went to weld the underside when the 100lb step fell off and landed on thumb.

Employee later experienced pain and went to see a specialist on 1-30-2018. Employer was notified on 1-30-2018 that the employee required physical therapy and would not be able to work until completed.

EE was diagnosed with lower extremity cellulitis and is currently losing time.

Employees were cribbing up a shuttle car to change a tire. The tire slid off the hub of the shuttle car, hit the ground and fell over on the employee breaking their leg in two places below the knee.

The employee put out left hand to try to catch self, and hurt left wrist when ee impacted the ground.

EE was moving a piece of flagstone with a partner when it slipped from their grip and fell, striking EE's foot. EE's left foot showed a small contusion on the top of EE's foot, above the steel-toe of EE's boot. X-ray revealed a non-displaced fracture of the second metatarsal. EE is non-weight-bearing on crutches. Modified duty will be assessed on Monday the 18th.

Employee was turning valve to dump pump box on re-grind mill and felt pain in shoulder. EE was seen by a first responder, requested further medical treatment, seen by PCP, taken off work until next shift rotation. Employee was helping change a track pin on an excavator. The employee injured was standing nearby. Another employee was hammering the pin out. Part of the pin split of and hit the injured employee in the eye. The shrapnel was embedded. 1st surgery 12/7/2017, second surgery 1/9/2018.

Employee was attempting to adjust airbrakes using slack adjuster on a Mack truck. Another employee was instructed to engage airbrakes to check adjustment. Employee finger was pinched between slack adjuster and frame. Employee was sent to emergency room received 20 stitches to right ring finger around nail area.

Injured got a foreign body in right eye and had to have it surgically removed. Injured also had prescription prescribed for the injury.

Employee will need surgery to repair the broken bone.

While doing clean up in front of the sag mill, employee was pulling a wash water hose to wash down the area. Later at home, moving EE's shoulder around EE felt grinding, and claimed it was due to work. This event was questionable, but after a full investigation, and receiving medical records which allows an injury in line with the work, the claim was accepted.

EE was doing an elevator chain and bucket inspection after the elevator had been repaired. EE was sitting on a bucket outside of the elevator, controlling the chain with an inverter and looking up when a 1in -rock became dislodged from the chain/bucket above and struck in the eye. EE was not wearing any eye protection.

Occurred on 3rd shift (10:00pm-6:00am). Employee changing screens in the METSO screen, pulling on a screen, felt a pain in shoulder. Diagnosed with tendinitis on 12/8, permitted to return to work, and could continue job as normal. The pain continued, follow up MRI found tear in the rotator cuff which was diagnosed on 1/16. This will require surgery and time off.

The employee was working near the raw mill when EE tripped on the catwalk but caught himself on the handrail. When EE caught self EE felt a strain in right small finger. We monitored the strain to see if it would improve but it didn't. A doctor diagnosed that the tendon in the finger had torn and needed repair. The employee had surgery on 3/22/18 making it reportable.

Working on a motor pulley, employee's hand was caught in a pinch point. Laceration to right, small (pinky) finger and broke the end of right, small (pinky) finger.

While installing saddle liner in 5 1/2 ft cone crusher, the part became wedged. While freeing the part it rolled back, pinning employee's LT hand between the part & the main frame of the crusher.

Employee stopped grinding and leaned forward. Dust/debris that had built up on hard hat or faceshield bracket, fell down EE's face and into EE's right eye. Eye was washed out & employee felt eye was okay. On 12/18/2017, employee reported that eye had gotten worse; red from irritation, causing pain and blurred vision. Employee was taken to Quick Care for foreign body removal.

An employee found a conveyor motor was turning but the gear reducer was not as the V-belt was slipping. The employee removed the guard and pulled the V-belt to add tension when the belt started and pulled ee's right index finger between the belt and pulley causing a laceration below the second knuckle resulting in 2 stitches and returning to work with no restrictions.

While the EE was changing the crawler mobile tailpiece valve bank, the individual got solsenic in eyes, nose, and mouth. The pumps were shut off and pressure was bled off of the system. The employee was wearing all required PPE at the time of the incident. The EE was properly trained in the task being performed.

Loader Operator began having neck pain approximately two weeks prior to reporting. Employee believes the symptoms are from ""bumping around in the loader.""

EE used a shovel to catch the empty drum and it fell again. After a couple attempts at grabbing the drum with the shovel, ee instinctively reached for the drum and cut finger on the sharp edge of the drum.

Two miners on the rehab crew were in the process of installing 5 foot split set rock bolts with a pneumatic drill from the deck of a scissor lift truck. When the crew bottomed out the hole, the drill steel became bound inside the hole. As the operator collapsed the leg, a large slab fell from the back striking the scissor truck causing injuries to the rehab crew.

EE and co-worker were carrying a cross tie. Co-worker dropped one end causing EE to drop the other end on right foot where steel toe ends. EE had x-ray and right toe was fractured. Foot was taped up. No work restrictions at this time.

The employee hit the left shin and suffered a contusion, treated with first aid. During the follow up appointment 12/6/2017, the start of an infection was discovered and the employee was prescribed antibiotics.

IE spun the steel slightly and IE's right glove became stuck causing it to twist IE's ring and pinky fingers on IE's right hand, resulting in fractures of both fingers.
