The BMX slope hoist was out of service due to a bad brake line for the hoist drum. The brake line was replaced and the hoist returned to service.

Several roof falls have occurred on the old worked out panel from spad #'s 6719, 6717, 6696, 6701, 6716 2nd crosscuts outby the old faces. Also at spads 6680 in #3 entry and spad #6699 in #2 entry. Supplemental support in the form of timbers and cribs were installed. All approaches have been dangered off.
The hoist drum hydraulic hose that controls the brake pressure developed a leak which caused the hoist to be inoperable for over 30 minutes. The hose was changed and the hoist returned to normal operations.
Loader operator noticed smoke coming from under feed hopper. Contacted plant operator. Plant operator called 911. Attempted to extinguish fire with fire extinguisher. Unsuccessful. Loader operator saw fire going up incline. Went to top of tower to extinguish with extinguisher. Unsuccessful and retreated to safe location.
A roof fall above anchorage was found by the weekly examiner in B Entry of the 7 seam B section. This fall was 90x16.5x7.The fall blocked the airway for travel but did not alter ventilation. The area was dangered off on all approaches. There were no injuries to this accident. This area was mined in Oct. 2014. The travelway was rerouted around the fall.
Hoist was down with `worn brake alarm / excess brake air gap' switch.
An anonymous caller informed MSHA there was a fire at stockpile #4. The flame was approximately 6 inches high and 2' by 2' in diameter. The pre-shifter spotted the flames and tamped them out with an excavator. The flames were out within 15 to 20 minutes.
Type V support 8ft. swellex and 16ft. super swellex. While installing the 8ft bolts in the back at the 3600 level of the Benbow decline, the bolter operator observed ground movement. The bolter was pulled back and shot-crete was applied. The operator then made ready the required 16ft long support, before installation of the 16ft. bolts the ground failed above the 8ft. bolts.
The Plant Manager was contacted by the Cross VFD stating that a fire had started in the ditch on County Line Rd in front of the quarry. The fire spread onto mine property before it was extinguished.
While hoisting a supply car with a shield loaded on it down the slope the supply car went off track 30' from the bottom of the slope. There was no damage but the hoist was out of service till the supply car was put back on track at 10:41 am.
When tested the safety brakes on the hoist car malfunctioned. The hoist car was removed from service and the brakes were repaired and the hoist was back in service at 11:25 AM.
Employee had just started a cut in the #5 entry on the #2 section with the continuous miner and as they raised the cutter heads close to the mine roof they saw a orange flame. No one was injured, power was de-energized, both agencies were notified.
The mine roof collapsed on top and outby the MRS as it was being moved. The fall measured approximately 8 feet in height, 25 feet in length and 19 feet in width. A clean up and retrieval plan was submitted to recover the MRS. No employees were involved and no ventilation controls were affected by this incident.
Two contractor employees were entrapped in the old finish mill passenger elevator on the 1st floor for 38 minutes. The first floor door would not open.
Non-emergency evacuation out of the mine due to a land line phone cable that broke free of its supports in the Koepe shaft. This phone line had to be removed so that the Koepe could move up and down the shaft.
Non-emergency evacuation due to North Skip track limit switch would not allow North Skip to move. The track limit switch was replaced.
An after market switch gate sensor failed causing the secondary escape (alimak) to become inoperable.
Elevator was down with electrical problems (blown fuses)
An ignition occurred on the H-3 section at approximately 5:55 pm while mining the #2 face.
An unintentional Roof fall occurred off section Break 53 in Secondary escapeway. The cause is unknown and there are no injuries.
An unintentional roof fall occurred in panel 4, entry 6 in crosscut 56 intersection. The roof fall measured 18'L x 12'W x 6'T. The roof fall will be cleaned up and re-supported according to approved site specific plan. The roof was supported with 5' fully grouted resin roof bolts. Dated of development was 12/17.
The cage hoist went down due to an electrical issue on the 01/16/2018. It was reported to mine management at 11:40 pm that it would not be back up within the 30 minute time frame and a call was made to the MSHA 1- 800 hotline at 11:50 pm to report it being down.
The roof fall occurred in the 1st south panel off the 1st main east in entry #12 between SS 41+60 and 42+20. The roof fall measured approximately 35' feet long, 18.75' feet wide, 9' to 10' feet high. This fall was partly in the examiners weekly examination route.
A weld broke on the bottom secondary door switch bracket which interfered with the normal operation of the elevator for more than 30 minutes.
A roof fall was discovered in the track entry between break 25 and 26 on # 2 belt. The fall is located between spad 6478 and 6481. Its approximately 20'long X 18' wide x 8 foot high.
Lost power to the entire site from the Cleveland Public Power electrical feed.
The Koepe would go up 5 feet and then stop. A non-emergency evacuation was initiated and all UG personal went up the production hoist. The air compressor was changed out from the west one to the east one. The brake caliper linkage was also lubricated.
An ignition source within the kiln coal dust collector created a change of pressure inside the baghouse causing the compression doors to blow open and the fire suppression system to activate.
On the morning of 1/12/2018 the area had a winter storm with ice and snow and the facility power kept flickering back and forth causing the PLC on the hoist unit to scramble its brain so to speak. The utility company supplying the power checked their lines and they discovered a line that had been hitting together.
An alleged face ignition occurred on the 7 North Mains Section - Left Side while mining at the face of the #2 Entry for the 1W longwall development section. There were no injuries or damage to equipment or mine structures as a result of the event.
The elevator at BMX portal was out of service due to a main contactor needing cleaned. The tips of the contactor were cleaned and the elevator returned to service.
Production hoist would not power up, computer is on with no faults showing. Upon trouble shooting it was found that the linkage on a contactor (in the lowering circuit) was loose.
An unplanned roof fall was discovered reportable at the 69 block intersection of Main North 3 Belt. The fall was approximately 25' wide, 30' long and 18' high. The fall was most likely caused by gradual deterioration of the intersection. No problems with mining machines, PPE, training or compliance issues.
An unplanned roof fall occurred in the 68 Head Gate section in the One South bleeder entry between 6 xc and 7 xc. The fall was 38' long 6' high and 7' wide. The fall went from center of entry to the rib. No one was injured.
A non-injury roof fall occurred in the #4 on the Southwest Mains at survey station #976. The fall measured approximately 20 feet wide by 8 feet high by 50 feet in length. It is not affecting or impairing ventilation. It is located in a return air course. We do not intend on cleaning the fall up. We are dangering it off. No person(s) or equipment was involved in the fall.
The driver was not aware of where the rear axle was when backing up to the dump location therefore backing into a hole that was approximately 8 foot deep.
A non-injury roof fall occurred in the #3 entry on the Southwest Mains at survey station #994. The fall measured approximately 20 feet wide by 8 feet high by 80 feet in length. It is not affecting or impairing ventilation. It is located in a return air course. We do not intend on cleaning the fall up. We are dangering it off. No person(s) or equipment was involved in the fall.
The door sensor strips malfunctioned causing a delay in the availability of the Pleasant Grove Elevator for more than 30 minutes. The malfunctioning door sensor strips were removed and replaced with new sensors.
A non-injury roof fall occurred on the #3 Belt Conveyor on Northwest Mains at survey station #546. The fall measured approximately 20 feet wide by 5 feet thick by 50 feet in length. A roof fall clean up plan has been submitted to MSHA and has been approved for clean up of the fall. This fall was immediately reported to MSHA. No person(s) were involved in the fall.
Due to the cold weather, the pump at the bottom of the Sparta shaft failed and water accumulated in the sump of the shaft causing the buffer switch to freeze.
The Crabapple elevator was not operational for an extended period of time. The inner door switch at the top of the elevator malfunctioned causing the elevator to not operate with automatic controls. This switch was changed to restore the elevator.
A roof fall occurred in the 2nd north off the 2nd main east in entry #6 at 67+40 SS at x-cut 86. The fall measured 20 feet long x 19 feet wide x 10 feet high. This fall occurred in the secondary escapeway.
The bearings came out of the shive wheel at the bottom of the elevator on the rope brake. It was causing a drag on the rope brake causing the brake to set. Lake Shore was called and they sent the parts to fix elevator. The elevator as fixed on 1-7-2018.
A fall of roof occurred on Unit # 2 , Entry # 7. The fall measured approximately 40 feet long x 28 feet wide and the thickness is unknown.
Hoist accumulated ice do to weather Drop in Temperature
The Dry Ridge portal Elevator was not operational for an extended period of time. Investigation revealed a frozen magnetic switch on the elevator doors. The switch was changed to repair.
Non-emergency evacuation from underground due to one of the production hoists computers could not reset a series of faults. Additional PM work was conducted during the non-emergency evacuation.
Roof fall on the longwall stageloader. This fall was 12' X 18' at the widest point. It separated at 7 1/2 foot level and laid down on the stageloader. It did not impede passage or block escapeway. MSHA was called and a plan was submitted and approved.
The Dry Ridge elevator was not functional for a extended period of time. A screw that holds the main contactor into place had loosened and fell between the tips. This screw would not let the contactor tips close. The screw was removed and replaced.
Portable heater caught fire while being fueled.
The elevator for the south portal was inoperable due to a piece of ice on a governor rope which caused the car brake to not release. The elevator was fixed after approximately 45 minutes.
A Leak was found in a 500 gal propane tank outside the office building. All personal were evacuated to the quarry main gate. The gate is about 1/4 mile from the office. Once the propane company had check and repaired the tank the all clear was given.
Employee was using a cutting torch to cut through 1/4"" steel on a UHMW-lined conveyor chute. The UHMW ignited and burned inside the chute, which caused the conveyor belt at the top of the chute to start on fire. Employee was unable to extinguish the fire with a fire extinguisher. The fire department was subsequently called to extinguish the fire.
Service hoist tripped out on a brake fault and could not be reset. Leaking air valve was identified during trouble shooting. Valve was repaired, test runs were performed and the hoist was put back into service.
An alleged face ignition was reported on the F28 TG section while mining in the #2 entry. An investigation revealed that small areas of pyrite were found in the rib and face areas. During mining these pyrite areas were struck and may have caused sparks that contributed to the face crews assessment that an ignition occurred. No violations were issued during the investigation.
Damage occurred to the on-board control circuit on the Lakeshore Man/Material Hoist at the beginning of day shift. An investigation revealed that the control cable that hangs beneath the cage pulled into due to the extreme cold temperatures (below 0 wind chill factor).
We had an unexpected roof fall above the anchorage point. We had 6' torque tension bolts and 10' cable bolts installed. All entries have been cribbed off and dangered off. The fall was located on #9 belt line at 13 block.
The E30 development section experienced a face ignition while mining in the #2 track entry. During the investigation MSHA cited two worn bits on the left side of the continuous miner head on the drum extension that they believe may have contributed to the ignition.
Multiple (3) tractor trailers on fire with tires in the trailers. Suspected vandalism.
Employee was applying heat with a natural gas torch to attempt to free up a frozen flop gate at the top of #3 stone silo. In the process of heating the flop gate, employee inadvertently ignited the adjacent S-132 stone conveyor belt.
An unintentional roof fall was found on 1/1/2018 at approximately 12:40am. The fall was in the # 2 entry inby spad #1473 and outby spad #1463. The approximate dimensions of the fall are 20' in length X 20' in width X 8' in thickness.
EE was fatally injured when the articulated haul truck EE was operating traveled off of a haul road into a lake at the Hallett Materials sand and gravel pit. The crew was stockpiling production material during the night shift when they noticed EE was missing. Crew members began searching for the missing EE and found EE's haul truck submerged in water.
Employee was in the process of changing an articulation jack. Inj had already installed the cylinder end and was in the process of installing the piston end. The piston end became lodged on the ears of the main frame of the machine and inj was re-positioning the piston end when it unexpectedly shifted resulting in a partial amputation of the tip of the right middle finger.
Employee was installing beam clamp with gear box rotated on chain fall pinning employees foot against C-Channel causing the ankle to be injured.
After loading belt structure into the back of a flatbed pick-up, employee had to adjust the structure to fit. Employee pulled on the structure and when it came loose it pinched a little finger between the structure frame and the pick-up bed resulting in an amputation of the distal phalange of the little finger.
Ee was spraying shotcrete. EE reported EE repositioned sprayer and was in process of setting up boom when EE slipped, bumped the joystick, causing boom to swing to left, pinching EE's right index finger between the light bracket on boom and the remote holder. Partial amputation of right index fingertip involving bone.
A fully trained miner was shoveling under the belt when the shovel contacted the bottom belt causing the shovel to pull their right arm and injure the shoulder. The accident was not the result of any compliance issues nor were any protective guards involved.
Employee was removing rollers and cradles from the #2 belt when EE felt a pain in back. The injury is being treated with physical therapy. Cause of injury was overexertion and improper lifting. Compliance with rules/regs,PPE,mining equip/systems not a factor. Miner proficiency may be a factor.
Repairing a tire on mantrip, using a 4lb hammer, ee missed rim & pinched tip of right index finger.
EE was bagging at the silo and a pipe wrench handle fell from above and hit EE in the head while EE was wearing hard hat.
The tires on the Komatsu 630 Haul Back Truck, were deflated first and while removing a lug nut, the tire fell on EE. Several lug nuts remained in place when tire suddenly fell.
Injured person is not a direct employee of Omega Surface Mining, LLC. EE is a truck driver employed by a contractor to haul coal from Omega's jobsite. EE came to the jobsite and got out of EE's truck to see if EE's truck bed was clear; EE slipped and fell on ice injuring EE's left hip.
Miner was operating a skid steer loader. Operator was exiting machine too fast, lost grip and balanced and fell back in to seat. The operator was taken to a doctors office. The operator dislocated finger and bruised ribs. To prevent this injury in the future the operator was advised to exit the machine slower and more carefully. Operator can return to work Monday.
A fully trained miner injured the left middle finger while roof bolting. Employee was cleaning rock off the miner when a piece of rock fell from the top hitting the finger. The accident was not the result of any compliance issues nor were any protective guards involved.
While removing a 90 pound brake from shuttle car, employee's hand was hit and received lacerations.
Helping electrician weld a hold down roller for the #4 belt drive. The roller slipped and fell on employees foot.
IE WAS CLIMBING UP ON DOZER AND GRABBED THE HANDRAIL AS EE STEPPED UP FOOT SLIPPED OFF THE STEP PULLING ON EE'S ARM.
Employee stated EE was shoveling material onto the belt when EE felt a pop in the right shoulder. Employee sought medical attention on 1-30-2018 and was placed off work.
Employee was tightening a fitting on a hydraulic hose when they felt a pop in left elbow.
Employee was walking across railroad tracks in the rail yard. As the employee crossed the final track on the way back to the truck, employee fell and hit head.
Employee was shoveling on tail piece when they felt bad pain in the middle of their back.
The employee pinched EE's right ring finger while operating the supply tractor
Injured was installing side boards on #10 Shuttle Car in the St Leo working section. EE was in the bed of the shuttle car and when attempting to dismount the shuttle car (to go and get parts) EE twisted left knee when stepping down from the bed of the shuttle car down to the mine floor, resulting in a strain/sprain.
Employee was stepping off the last step of a 3' ladder and stepped onto a large metal object rolling EE's foot slightly causing a fracture to EE's little toe.
Employee was installing a new bottom roller on East belt and as they were lifting the roller they felt pain in lower back causing injury.
Worker was on top of bag-house inspecting bags through open portal. Cover of portal is removable and was leaning against guardrail for portal access. Wind caused cover to fall back over portal. Worker was struck by falling cover on the right arm/elbow. Worker was unable to continue duties due to pain and limited mobility of right arm/elbow.
Employee was walking across the parking lot from car to the bathhouse when EE was struck by a vehicle driven by another employee at #9 Portal.
Employee removed a top belt roller to make a splice in the conveyor belt. When ee turned to drop the roller ee felt a pain in right upper arm. EE received medical treatment on 3/29/2018 and returned to work on next scheduled shift on 4/3/2018 and begin physical therapy on 4/5/2018.
The employee was using a bar to help remove a bridge in a transfer chute and suffered a strained back.
Employee was placing the trolley pole back onto the trolley wire when a rock fell from the mine roof and struck the employee in the right side of head and back.
Report was, while retrieving a bolt from the miner, a piece of rock fell from the roof and struck ee on the head and back of the neck. Initial doctors slips and evaluation reports were not consistent with this incident description. An independent medical evaluation revealed that the symptoms were related to the incident description, therefore this incident became a LTA on 3/28/18
On 1/26/18, employee informs of a severe chest pain. We proceed to transport the employee for medical treatment. On 1/29/18, the employee proceeds to visit EE's personal MD, who recommended work rest. On 2/1/18, employee go to the Emerg room do to continue chest pain, where EE was evaluated and diagnostic muscular sprain work related and was referred to workers compensation.
Employee was pulling miner cable when a rock fell on left ankle. Took to Pikeville Medical Center for evaluation of injured ankle, x-ray showed no major injury, nothing broken. Employee received 3 days off - paid by employer - Saturday, Monday, & Tuesday.
Employee was backing up dozer, slipped off rock jolted the employee
Employee was walking around a 2 yard loader when employee slipped and fell to the ground. In an attempt to catch self employee had both arms extended out to brace self for the fall. Seen by site medic, referred to ER, x-rays were positive for a fracture in the right humerus, restrictions were issued.
Employee stepped down onto a rock turning EE's right ankle while getting off Mega Track resulting in a very bad sprain.
On 1/25/18, EE was cleaning feeder tube and twisted knee when EE stepped up onto a flange. This incident became reportable due to lost time as a result of knee surgery performed on 7/5/18.
Employee stepped on rock in roadway twisting the left ankle resulting in a very bad sprain.
Employee was raising a vent tube to secure it to the roof and felt a pain in EE's groin. EE worked all scheduled shifts until diagnosed with a hernia and began missing time for surgical repair on 2/19/2018
Employee was helping operator change bits on the tailgate side of the shearer when employee was struck by middleman that rolled out pinning employee's leg to coal in face chain causing employee to turn left ankle.
Employee was standing on a work platform approx. 18"" high building a block wall. The platform legs buckled on one end causing platform to tilt and employee fell to the mine floor. While trying to keep balance ee twisted right knee. Employee has been diagnosed with a torn meniscus. Employee didn't violate any regulations was wearing appropriate PPE and has proper training.
Employee was exiting pickup truck at the end of shift. As employee exited the vehicle, right foot got entangled in a cord that was hanging down by the brake pedal. The employee fell to the ground as employee exited the vehicle.
Lifting drill steel and felt sharp pain in lower back/hip area
Employee was using a breaker bar to remove a bolt on the wheel assembly on a Komatsu WA 800 when EE felt a pop in lower groin. EE was diagnosed with a hernia. EE remained on regular duty and this did not become an MSHA reportable injury until EE had surgery on 02/16/2018.
A piece of rock fell from the rib and struck employees right leg. This caused a fracture to the right heel bone.
When throwing a crib block under a shuttle car ee smashed right ring finger between the crib block and the shuttle car frame. This accident caused a laceration to finger and broken bone in the finger also.
Employee was changing idlers on lines 8 and 9 of the CF2 with three other mechanics. A pry bar slid down the ramped walkway and struck the employee in the left gluteus causing a bruise.
Victim was injured when a scoop rolled forward resulting in victims left leg receiving trauma and fractured fibula, prior to being pulled from under the scoop.
Employee was installing wire mesh when a rock fell out between rib and wire mesh hitting employee on the left foot and ankle.
At the completion of a job, EE's left arm got caught by a belt roller on the plant feed belt causing abrasions. EE attempted to knock belt skirting down against the belt with a hammer and missed, allowing coat sleeve to be caught between the top roller and the belt. Transported to Springfield Memorial Medical ER for treatment where EE was admitted for observation.
The employee was operating the Brokk machine into the kiln. The ramp was pushed off the discharge lip of the kiln causing the employee to fall into the cooler area below. The employee sustained fractures to the pelvis, vertebra, and ribs. As well as internal damage to the rectal area.
While mining 4-5 crosscut at 97 block St Leo the return side bolter operator, on an ABM14, had hand on top of rib door while attempting to extend the slider ventilation tube with right hand when a rock fell from behind the tube and struck the posterior portion of wrist and metacarpal area of gloved left hand, resulting in a contusion.
Employee accidentally ingested diesel fuel. The employee was diagnosed with toxic effect of chlorofluorocarbons, accidental (unintentional). This is a degree 7, classification 24 occupational illness.
Employee was climbing down off 777 Truck when EE stepped off the step and right foot hit soft ground and EE twisted right leg and foot. EE has worked every day since but was put off by Doctor 02/22/18.
Employee was climbing down off a D11 bulldozer and slipped on the muddy track, when EE slipped EE pulled something in the shoulder. EE was released to full duty that day but after a CScan in June they found a slight tear in EE's shoulder.
Employee was making EE's examination at the camp creek hoist when EE slipped causing EE to twisted and hit EE's left knee. Employee did not start missing time due to this injury until 02/12/2018.
HELPING CHANGE THE HOIST ROPE ON THE 7450 DRAGLINE. WAS REMOVING THE WEDGE FROM THE HOIST CABLE SOCKET USING A RUBBER SLEDGE HAMMER WHEN EMPLOYEE INJURED ARM WHILE SWINGING.
Injured was retrieving a cap board near the section power car in St Leo when rock and coal fell out of the roof. The injured turned and ran, but a piece of rock/coal struck EE's left knee resulting in a sprain/strain injury from the impact of the falling rock combined with twisting at the knee to run. Employee did not start missing work until 2/21/18.
employee was caught between a long wall roof support and the AFC panline as EE was attempting to position the roof support into place on the face, during a long wall move
During pumpable crib installation, the employee was exposed to water during the shift. This caused burns on both feet from the concrete used during the process. The employee began missing work on 1/23/18.
While performing a pre-trip inspection of a mantrip IE fell to the ground and hit knee. IE was seen by a physician on 01/25/2018 and continued working with a full release. IE was diagnosed with a torn meniscus on 02/09/2018 and was restricted from returning to work until after surgery.
Employee was removing crusher liners with a double jack and felt pain in the lower back.
An employee was exiting a work area when the SST hose struck EE in the back of the legs causing EE to fall on left side. The employee injured left ankle.
Miner slipped on ice and felt a pop in left arm.
EE was walking out of zero gate when the EE twisted the right ankle. EE did not disclose the incident or have incident report filled out until 1/23/18 with complaints of pain to the ankle.
While standing beside EE's ride, a piece of rock fell and hit EE on the neck/shoulder. Employee worked until 10/22/19 when EE began missing work. This was recently diagnosed as a degenerative disk disease.
Mechanic was helping install tracks back on a drill, watching the track and idler for clearance, EE heard a pop, then felt something hit above left eye. EE didn't realize was hurt until EE wiped face and saw blood. After EE got home, felt something there, went to PCP and a small piece of metal was removed. PCP gave EE a day off work.
Employee was moving 50lb bags by hand and felt a strain in the back.
While pre-shifting and walking the injured slipped and fell and caught self on Right arm. EE felt burning pain in Right shoulder. Was assessed in E. R. by Dr. as strained shoulder muscle.
Employee was loading EE's personal back pack into the back of our company pick-up truck the rock (ballast) slid and EE injured EE's right knee. The ballast was on a slight incline, and lose which caused EE to slip.
Employee was roof bolting in the #4 face when a piece of rock fell from the mine roof knocking off hard hat. As the employee turned to get hat a second piece fell from the mine roof striking the employee in the top of the head causing a laceration. The employee went to Raleigh General Hospital and received 3 staples. Employee was off work for 1 day.
Employee was dismounting truck and when foot hit the ground, EE felt knee pop.
Employee was backing up loader from the muck pile and the left rear tire of the loader stood up a blasted rebar bolt which contacted the employee on the back side of right leg near the knee causing a contusion. Seen by site medic, referred to ER were restrictions were issued.
As employee was getting out of the scoop operating compartment, they felt pain in the left elbow/arm. Employee began missing time on 01/19/2018.
The rock breaker was breaking oversize down small enough to run to primary crusher. A rock came back and hit the cylinder and then ricocheted back toward the cab entering the cab hitting employee in the right side of the head.
EE bent over to pick up a 3 inch plastic discharge pipe and felt pain in EE's lower back.
A fully trained miner was installing a rib bolt when the ventilation slider came out of the other ventilation tube striking EE on the right shoulder. The accident was not the result of any compliance issues nor were any protective guards involved.
Employee was walking between two buildings on site when employee slipped and fell, breaking the fall with employee's left hand/elbow. This resulted in a fracture of the left humerus (upper left arm).
Employee was lifting the door on loader to check the transmission fluid and as ee pulled the door, it got stuck causing to strain to open it. This resulted in a hernia.
Ice had built up on a bottom roller of the clean coal collecting belt. IE (temporary employee)struck the bottom roller with a hammer to dislodge the ice buildup. IE's left hand was caught between the bottom roller and the belt. IE received a fractured left humerus.
Employee stated was filling a bulk bag and as employee was pushing on the bag felt pain in front left shoulder. Employee was seen by a physician and placed on light duty.
Employee went to pull cable bolt on ground to put into roof. Cable bolt was hung on a pile of coal and when ee snatched it hard ee felt a pop in left shoulder.
Employee was repairing a railcar coupling, weighing about 200 to 250 lbs. EE picked up the coupling several times and as EE rotated or twisted EE's back EE strained/sprained EE's lower back. Took self to the hospital the following day. EE missed two days of work and is now back to work on 01/19/2018.
IE was working on construction of a vertical shaft. IE was in the process of flipping a bucket bail, on the muck bucket, to the open position and was struck in the foot by the bucket bail. Proper procedure was not followed as the bail stop was not activated prior to flipping the bail locking ears.
Employee was standing on the step on the side of the CM tightening a chain on a vent tube. Employee's right hand was positioned at the base of the rib drill and right elbow contacted the 'feed' lever causing the bolter head to move and pinch employee's right index & middle finger between the frame and bolter head. Had surgery to repair the tip of right index finger.
While descending the 972K loader, the miner missed the last step. EE's hand slipped off rail causing a fall on left shoulder. Refused to be taken to hospital and went home. EE said would see the doctor from home. On April 13, 2018 miner had rotator cuff surgery.
Contract employee was attempting to remove a cover on a piece of equipment when they felt discomfort in the left shoulder area.
A fully trained miner was walking in the supply yard when the miner slipped on ice and when the miner tried to catch self by grabbing a handle on the machine felt a strain in right arm. The accident was not the result of any compliance issues nor were any protective guards involved.
The employee was going to run an articulating truck for the shift. The employee was trying to clean frost off the passenger's side mirror, employee stepped out on a compartment lid on the fender, slipped and fell landing on employees lower back and buttocks.
Employee was cleaning off a shuttle car with back towards the scoop. Scoop man was rock dusting outby areas. EE backed up to the shuttle car never seeing the employee cleaning off the SC. EE accidentally pinned the left leg of the injured employee between the SC and scoop.
Employee was shoveling snow when EE slipped on ice, fell and hit EE's head on the ground.
Setting caterpillar water pump, the fill pipe swung and struck employee in the shoulder/neck area.
Was unloading an acetylene tank when it slipped out of the employee's hands. When EE tried to catch it to regrip the tank, EE felt something pull in left arm where the bicep and elbow meet.
Employee was operating doghouse between shift, while shear was going to tailgate roof bolt was hung in the drum. The bolt came out of the drum on the chain and struck EE in the face causing lacerations on top lip and chin and knocking out tooth.
The employee was removing a pump casing to perform preventative maintenance. When the employee removed the casing, EE's hand was inside the pump. The pump shifted and EE pinched EE's right index finger between the pump case and impeller. This resulted in a break at the tip of EE's right finger and a laceration requiring several stitches. The employee was wearing gloves.
Employee was hanging miner cable and felt a pain in EE's stomach. EE continued working until EE had hernia surgery on January 25, 2018.
Employee was walking on the surface of an underground mine to gather materials for EE's shift. As EE was walking, EE slipped on uneven surface that was slick from snow causing EE to fall to the ground. As EE fell EE caused a fracture to EE's left leg/ankle. EE began missing time on 01-16-18.
EE was moving car anchor & pulling cable off of reel when ee felt sharp pain in lower back. EE was released back to work 1/17/18. EE returned with no restrictions on 3/19/18.
Employee slipped and fell to the ground injuring right shoulder.
A miner was walking in the parking lot when EE was bumped by the right front side of a pickup truck traveling at a low rate of speed causing EE to fall and strike the back of head. EE was taken by ambulance to a local hospital of EE's choosing and was treated for a laceration to the back of the head that required 10 staples and was released.
While attempting to free ice from a bottom roller on the refuse stacker belt, employee's left hand/arm came in contact with the bottom roller and became caught between the roller and bottom belt. Employee sustained burns to left forearm and required an incision to relieve swelling.
A fully trained employee was loading belt structure when the employee caught left pinky finger between a piece of structure and a roller. The accident was not the result of any compliance issues nor were any protective guards involved.
Employee was installing a backboard on the end of a rail car when ee slipped on a track tie injuring left knee. EE was later diagnosed with a partial tear of ACL/MCL in left knee.
Moving power in 5E #2 entry near 10 blk with scoop pulling power train (power car, high voltage tub, then E-sled). Injured went to hang high voltage cable when the tire of the E-sled rolled onto left foot, resulting in contusions to foot and a laceration on left shin that required 4 sutures to close.
Employee was lifting a lid that had fallen off of the bolt machine, to put it back on. The lid weighed approximately 70 lbs. Employee twisted their back trying to lift the lid alone. Employee was taken to the hospital and has not returned to work.
Moving a Diaphram water pump in the shaft of the mine, standing on uneven ground, when lifting pump, right knee give out.
Hanging mud flaps on the right rear of a 777H cat rock truck. Slipped off a ladder jammed the heel of right foot in the ground felt a sharp pain go down the back of leg behind the right knee.
A fully trained miner injured left shoulder when the mantrip which the miner was a passenger in struck another mantrip. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was bolting in #5 entry at Spad # 17+60 and as employee was walking back to pull up the bolter, ee hung the right foot under a rock lying on the bottom, tripping ee causing ee to fall twisting ankle as ee fell to the ground. Employee finished the shift and didn't go to the Doctor until that afternoon.
A fully trained miner was installing a rib strap when a piece of rock fell from between the roof straps striking right shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was crossing a crossover at 3A tailpiece. Employee was dusting around 3A drive backing off of cat walk and fell. Employee was complaining about stomach, right elbow, right leg, low back, and tailbone.
A fully trained miner injured right hip when the mantrip that they were driving struck another mantrip. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was passenger in straight truck that tipped over on its side due to poor and muddy road conditions resulting in employee bruising left shoulder and a contusion to forehead.
Electrician was re-energizing a 480V breaker when an arc flash occurred. Site contacted MSHA as the extent of the injuries were unknown at the time of incident. After arriving at UNM, injuries were determined to be less severe. Employee was diagnosed with 2nd degrees on 3% of body.
Injured EE was carrying a belt roller on EE's right shoulder in the Prep. Plant. While walking around a corner, the end of the roller hit the corner and fell off EE's shoulder causing EE's right hand to bend and twist backward. EE continued to work up to 1/25/18, where EE had a x-ray and it was determined EE had a fracture in EE's hand. The employee then began missing work.

Employee reached over their head to turn a 6"" waterline shutoff valve and felt pain in the lower right abdomen. Was diagnosed with a hernia.

Our employee is a truck driver. Had just finished dumping coal, went to walk back to push in tailgate valve to relock the tailgate, when employee slipped in mud and fell on left side. Driver did not wish to seek medical attention immediately. Thought had just bruised side.
While lifting a bottom roller by its handle EE felt a pain in EE's left arm. Original incident 1/10/2018 - first day missed 2/14/18.
1-10-18 employee reported while connecting two 4 inch lines together they felt a pop on the right side of the abdomen. Seen by site medic, referred to ER & diagnosed with a hernia that appeared to be related to a recent Personal off work medical procedure. 1-22-18 seen by a surgeon & was determined the hernia was not related to previous procedure making it reportable.
Employee was lifting a bottom V-belt roller and felt a pop in right shoulder.
Contract employee was standing below area where square sets were being leveled when a loose piece of steel struck right shoulder from above.
As ee was stepping up onto bolting platform, ee lost footing causing ee to scrape right shin on the frame of the step. EE continued working without restrictions performing normal occupation. On 01/17/18 ee was referred to physical therapy and placed on restrictions that prevented ee from performing normal occupation, ee began missing time on 01/17/18.
Employee stated was attempting to untangle miner water line. While doing so the water line came in contact with a wooden track tie leaning against the mine rib knocking it over striking the employee in the hard hat and forehead area.
At approximately 2:15 pm, EE walked into office, bleeding from EE's head. EE had been outside washing down the concrete slab under the plant. EE lifted up and hit EE's head on the chute or part of the screw. EE has a gash on EE's forehead, was bleeding, 911 was called, ambulance picked EE up and transported EE to Fort Walton hospital.
Employee stated EE was tramming the bolter and hit a hole causing EE to be thrown into the canopy striking EE's head against the canopy. Employee was taken off work until 1/12/2018
While taking the front hood off a D11 tractor EE bent EE's head sideways looking under the hood and felt a pop in EE's neck.
After finishing the shift on 1/8/2018 a continuous miner operator said IE's back was sore. IE said there was not a specific incident but related the soreness to moving continuous miner cable throughout the shift. IE said the soreness escalated after the shift. IE has been diagnosed with a strained ligament in IE's lower back and began missing work.
Mine Operator was cutting in 9R and recognized the roof was falling out. EE radioed the section supervisor to come take a look at the roof. EE saw the IE walking up behind and continued to operate the machine. When ee turned around, ee saw the IE on the ground. A rock measuring 18""x40""x3"" fell from the roof from approximately 11 6 striking the employee on the head and neck.
Employee was holding a door back from slamming and rolled an ankle. EE continued to work and did not seek medical attention until 8/7/18.
The employee was pushing a roof bolt into the hole that EE just drilled when they heard something pop in their shoulder. The employee was injured on 1-8-18 continued to work until 1-16-18.
Employee was starting to enter the hole area for the 7' pipe, when employee stepped on ice and fell backwards onto the ground. As a result of the fall, employee broke EE's left femur at EE's hip joint, which required surgery to correct.
Sudden impact to both hands, resulting in numbness and pain.
Two Maintenance Techs were working to replace screens in screener. Techs moved top ring section with lid (approx. 60 lb). One Tech felt sciatic pain in back and leg. Evaluated by occupational medical services, advised to return home, rest, not report for work, take a prescription muscle relaxer. Appointment made for orthopedic evaluation on Wednesday, January 17th.
The employee was recovering belt structure when their finger was pinched between a belt rail and the bed of a ram car.
Coal was frozen in bed. Temperature was 10 degrees. Trailer was not completely level when attempting to dump. Truck tipped over when bed was raised.
The employee was injured when a section of pipe shifted when a track hoe caught the pipe. The pipe struck employee's leg causing a tibia plateau fracture as well as a small laceration next to the left eye which required stitches.
Bridge op side cat was up on employee's foot.
Employee's glove was caught by clips on stacking conveyor and pulled into the pinch point between conveyor belt and stacking platform.
EE was building a stopping using 6"" solid concrete blocks. While lifting a block and placing it on the 8th row, EE felt a pain in lower abdomen. EE later went to a surgeon who found a hernia in EE's groin. This will require surgery. EE began missing work on 01/09/2018.
After doing a pre-op inspection of HTK 262 operator went to pick up chock blocks and sprained their back.
Cold weather contributed to frozen material in a hopper resulting in the under hopper conveyor to be inoperable. The employee climbed on the machine to investigate and slipped and fell to the ground resulting in shoulder contusion
The employee slipped on snow/ice. Employee grabbed a piece of equipment to prevent falling and injured the right shoulder in the process. The employee did not receive ""medical treatment"" until 5/9/18.
EE was working in our yard stacking some stone on a pallet when a loose piece of stone fell down on foot.
A contractor Employee stated was sliding track ties under the rail when the rail dropped pinching left ring finger. EE applied a rag to the wound and refused further 1st aid. EE was transported to the surface to waiting ambulance and then to Raleigh Gen. Hosp ER where EE received stitches. EE remains off work. EE was not wearing gloves at the time.
Employee was walking while moving power and stepped on a rock covered in rock dust in the slant causing ee to sprain right ankle.
The employee was welding in a chute on approximately 1/3/18. The snow and ice that melted as a result of the welding refroze on the ground. The employee slipped and fell on right elbow, injuring right elbow and right shoulder. The employee's first day of lost time did not occur until 6/14/18.
At approx. 6:25 am the manager was informed of an accident involving an EE operating a front end loader. A truck driver reported over the CB radio that the operator was on the ground bleeding from a head injury. Evidence suggests the EE was cleaning a mirror outside the cab and fell off the platform just under 6 feet.
Employee chasing threads on shovel keeper bail on vise on service truck. Wrench slipped and employee fell to the ground, on shoulder. Initially refused treatment; 1/8/18 requested to be seen at Orthopedic clinic, taken off work pending MRI, results rec'd 1/12/18, referred for surgical repair. ONGOING.
Employee bent down and grabbed a bed rail and tossed in out of the roadway and felt pain in lower back.
While troubleshooting a bad roller, employee was attempting to move away from belt structure when employee's coat got caught by the belt. That pulled employee's arm to the roller resulting in a laceration to left arm.
Picked up a chain fall and felt pain in back. *EE continued to work as a belt man with no restrictions. EE had surgery on March 26, 2018.
Employee was operating the tail locomotive while moving a power center. The power center derailed and caught a discharge line connected to an air pump. The pump was pulled into the operator's compartment striking the employee in the back. There was no lack of job skills, training, PPE, or equipment protective devices.
Employee was loading rock into a basket when one rolled back and smashed left thumb.
Employee was washing down at the slope tail roller when they slipped and twisted left knee. Employee did not start missing time from this injury until February 28, 2018.
Employee was installing the last inside bolt in 5-right, when the canopy of the roofbolter came down striking in the head.
EE was finished placing bag ears on a forklift when EE's coworker was positioning the forklift forks to raise the bag to be dumped. The EE slipped and grabbed the mast forklift and caught hand in the forklift mechanism. EE sustained fractures and lacerations.
An electrician was working when a grease line pressurized by an air pump blew striking the hand and covering employee in grease. The employee was evaluated and released. On 2/8/2018, the employee returned to work from scheduled days off. On 2/11/2018, the employee's duties were modified to accommodate the continued pain in the hand.
EE was installing conveyor belt idlers with three other mechanics, during the installing the EE began to feel heat in the stomach area. Upon completion of the work RR felt discomfort. EE was transported to clinic and diagnosed with a umbilical hernia.
While cutting belt, employee cut left thumb with razor knife.
Employee was climbing down the loader ladder when EE's right foot slipped causing EE to fall on EE's right knee.
Maintenance on crusher track. Putting track back together using a pry bar. Pry bar slipped and hand was smashed between the pry bar and a skid steer bucket.
Employee bent over to tighten hose clamp. When ee stood up ee felt pain in back.
Injured was attempting to get product to flow from silo into truck using a bar when ee felt pain in shoulder. Incident occurred on 1/29, reported initially on 1/31, and requested medical attention on 2/5 when condition didn't improve.
EE caught toe in pallet lost balance and used right arm to catch themselves from falling to ground on 01/27/18, incident was first aid case. 04/02/18 Workers Comp was contacted (#201802962) due to limited progress on shoulder. Upon further review from medical advisers this became a Recordable/Lost Time as of 06/19/18.
Injury became reportable on 04/02/18 when EE had surgery. Employee was digging a headline cable that was buried in the sand and did not release pressure on the winch. Employee went to step over the cable, when it released from the sand it caught EE's leg it flipped EE. EE landed on right shoulder.
Employee was stepping down onto a platform when employee twisted ankle. Employee was placed on restricted duty 02/15/2018 and will have surgery to repair an ankle fracture and torn ligament on 02/21/2018.
Injured was walking and twisted their knee after stepping awkwardly, continued to have pain. On 2/26/18 had an MRI and discovered has a medial meniscal radial tear in the left knee. Was placed on restrictions on this date.
Tripped over cord while performing housekeeping tasks.

Employee was using a 24"" crescent wrench to loosen a bolt securing the gearbox that was needing to be changed. The wrench slipped off the nut causing EE to lose balance and fall on to EE's rear end. Employee chose not to receive medical attention at this time.
Driver ran over bumps traveling in and out of the mine. The driver was driving an 18 wheel tractor trailer. Driver complains of back and leg pain and is under doctors care at this time.
Employee was opening and closing lids on trucks at load out. The catwalk used for access to lids hung up on a safety rail on the trailer. When the employee tried to pull the catwalk free the employee injured left elbow. Employee stated felt a sharp pain in left arm at the elbow.
Employee was dismounting Haul Truck and slipped on ladder losing footing hitting left check on handrail. Initial injury was 1/24/2018 at 14:30pm. Employee was seen by physician and released to full duty with no restriction. Employee continued to have headaches and neck pain. Employee was taken to an Occupational Therapist for further treatment and placed on restricted duty.
Plant operator starting up the plant, noticed the main crusher vibrator not working. Locked out and proceeded to check if drive belt missing. Belt was still present, so EE gave the pulley a tug, felt a sharp pain in left shoulder for aprox 20 min. pain faded and worked rest of day, notifying management at end of the day.
employee was participating in voluntary stretching exercises during an ergonomic training class and sprain a muscle in lower back.
Reported 1/30/19 - Proceeded to back up the drill, the rear of the drill dipped sharply down, causing the front of the drill to jump into the air, throwing employee into the control console, woke up the next morning with pain in the right knee
Employee fell and hit head in January nothing said about knee. On 7/20/18 employee said ee bumped knee on a pallet but was ok. Employee went to Dr. 10/4/18 Dr. said ee has a torn meniscus and Dr. back dated to 1/23/18 for knee injury.
Team member was shoveling 4"" rock when team member's right foot slipped and twisted when the rock under foot shifted causing pain in team member's right knee.
Employee was putting buckets on the elevator and a piece of ware plate broke loose from above which fell to the bottom landing on EE's leg/ankle causing a compound fracture and breaking the leg/ankle.
Employee was walking behind the screen plant making rounds after taking over shift and stepped onto a raised cement barrier and slipped.
Employee was using a shovel to clean frozen sand near the Utica 2 Lippmann and felt a pull in groin area.
The employee slipped while descending a loader and pulled shoulder muscle. EE did not receive medical treatment at the time. The employee later went through shoulder surgery, which made the injury reportable. We are meeting the 10 day MSHA requirement.
Mill Operator injured left thumb while operating a hand crank to manually open a roll-up door.
An employee stepped up to access a screen deck and felt a pop and then pain in their left knee.
While descending boarding ladder of haul truck 206, employee slipped and hit back against the ladder.
Taking an empty bag and filling with 50 lbs limestone product, lifting full bag off a table (waist height) and pivoting 180 degrees, bending to place at ground level onto a wood pallet. Repeat process until pallet is 10 rows high of full bags (shoulder height). Injury to neck is undetermined at this time.
EE was installing a new disconnect box in the Sky Brothers MCCH3 trailer and was using a hydraulic hole punch in an energized panel and the hydraulic hole punch fell and contacted the live voltage that was not locked out and/or de-energized. EE received 1st, 2nd and 3rd degree burns to 14% of their body, mostly on the right side, also hands and left thigh.
While stepping down from the mounting ladder on a drill to the ground, the employee started to turn and walk away from the drill. EE's right foot stayed stationary and the rest of body turned in the direction EE was turning causing pain to right knee and right hip. Employee did not fall. Employee's first day away from work was 1/24/18.
While making a turn in the Quarry, the water truck tipped over.
Welding back side of hopper frame, EE was on one side of hopper, another EE on the other side of the hopper. There was frozen material build-up on catwalk of hopper and scrap steel in the area. EE slipped and fell to the ground.
Employee arrived at work, got out of EE's truck and was walking to the coal loader, slipped on some ice and fell. Employee caught self with EE's elbow jamming EE's left shoulder.
Ee was dismounting grader to continue pre-shift inspection. EE lost grip with right hand, lost balance, and fell to ground from 3rd step on ladder. Seen at ER where ee was transported for surgery on an open fracture to right ankle.
A Turn Bin MTM was exiting the lunch room returning to work and descended the wooden steps with right hand on the hand railing. The employee slipped on the 2nd step from the top and continued to slip down the remaining 6 steps, strained shoulder as employee held to the handrail.
Miner was closing the door to the crusher. While closing the door EE's hand was caught between the door and a latch.
Employees were replacing feed pump on ball mill, and while putting v-belts back on, employee got 4th and 5th fingertips caught between belt and motor sheave, fracturing them. Employee was given restrictions the company is unable to accommodate. Currently ongoing.
Employee was replacing batteries on a track backhoe. EE slipped on the track and the battery fell on hand, causing the injury.
EE was in EE's haul truck getting loaded. A boulder was in the dipper. As the shovel operator was loading the truck a boulder came into contact with the bed of the haul truck causing the ee to get jarred. Stated that EE's back hurt.
EE was helping move a de-watering pump and when hooking the insulated hook and cable onto the rib mesh, ee felt an electrical shock. EE finished shift and had pain in right forearm. No evidence of skin damage.
On 1-10-18 an employee was fusing pipe, when a fuse broke and raffinate started spraying. Employee moved quickly away when ee felt left knee pop. Felt ok, went home and started hurting. Came to work on 1-11-18, started hurting more called supervisor.
Employee was using hammer to work on trailer gates (sprained shoulder).
EE was lifting 100 lb. bags off of the ground and restacking them. While doing so, tripped on some bags on the ground possibly straining their back. The repetitive lifting and the trip caused them to have a sore back. The next day their back was having spasms and was in pain. Employee was diagnosed with a back strain on 01/11/2018.
While removing a vehicle wheel from a stack of 4, the wheel slid and pinched the employee's right index finger.
Stepping down out of the truck, slipped & fell to the ground, hurt back & elbow
While operating a rock splitter, employee reached for a rock that had rolled over the splitter rollers and felt a sharp pain in shoulder
Employee was assisting in unloading iron beams. piece of metal slipped and hit them on the top of the right forearm. Causing a bruise to the arm.
THE EMPLOYEE WAS CLEAINING DIRT OFF AROUND THE TAIL PULLEY OF A CONVERYOR BELT. THE PLANT WAS POWERED OFF, LOCKED OUT AND TAGGED OUT. THE BELT UNEXPECTEDLY ROLLED BACKWARD, CAUGHT THE EMPLOYEES GLOVE WHICH PULLED THEIR HAND AND ARM UNDER AND AROUND THE TAIL PULLEY. ANOTHER EMPLOYEE WAS ABLE TO GET A WRENCH AND ROLL THE TAIL PULLEY FORWARD AND RELEASE THE HAND AND ARM.
Employee was walking on clean hard surfaces (asphalt & concrete) from one end of the plant to the other and slipped on ice when between the pugmill and concrete silos. Medical Treatment for Ankle Fracture.
Employee reported slipping and falling on 01-06-2018. EE refused medical treatment and said was fine. On 3-1-2018 employee stated that the shoulder has not gotten better. We took EE to the doctor where they said EE had a contusion. EE was put on restricted duty and prescribed PT.
While roof bolting and removing a steel from the roof, employee was lining the steel up with the drill pot when a piece of shale struck employee's hand mashing it against the pot. The impact caused a fracture and laceration to the right index finger.
EE was swinging sledge hammer, ee slipped and hit hand on flange and dislocated socket on right hand/pinky finger
The Injured Worker(IW)was kneeling on the ground next to a I-beam(A) that measured 15'(long) by 15""(height) and had a 6"" flange. IW was welding plate onto the I-beam(A). Two workers were moving a like sized I-beam(B) next to I-beam A. I-beam B struck I-beam A causing I-beam A to fall onto IW. The impact caused a fracture to the left fibula.
EE had finished installing the trammel frame bolts. EE stepped out of the discharge box onto the mill deck on the southwest side. The grating shifted and the ee fell through the grating. EE scraped both shins. EE fell up to thighs, the ee caught self with hands.
Employee was climbing up ladder on 773 Haul Truck and felt a pop in right knee. Employee asked to seek medical treatment on 02/07/18 and was diagnosed with a strain.
Experiencing numbness, tingling, and loss of strength in right hand while bagging on Jet Mill 3.
Employee was filling the trickle duster with rock dust and some dust flew up into employee's eye
Employee was inside of kiln #3 standing on a wooden board work platform in order to descale the side of the kiln. One of the planks broke. Employee fell approximately 3 1/2 feet landing their back on top of rocky scaled material causing a fracture to a vertebrae.
EE was clearing a chute in the 40/70 tunnel and as EE struck the chute the bar EE was using bounced back and struck nose. EE was taken to medical and prescription for pain meds was issued.
Employee was inspecting fire extinguishers. Employee walked inside of train locomotive and stepped into floor hatch, injuring left leg. The floor hatch was left open by employees to allow heat to get to air lines. Employee did not use barricade or signage.
Employee's back became stiff and sore following working all shift hanging Huesker mesh on the longwall recovery face.
While cleaning the head of the dredge with a pry bar EE felt a pulling and burning sensation in EE's lower back
Employee was placing a board under a circular steel piece suspended by a crane when the board caught on the edge of the load causing a crushing injury to employee's left hand.
While lifting power cable by hand EE felt a pulling and burning in EE's lower back
EE was removing damaged bags of finished product from the 2AP conveyor. EE stepped backward while holding a 50# in hands. EE tripped and fell to the floor and the bag landed on right shoulder. Note - EE shift started at 6P on 12/31/2017.
While removing back plate covering screen, after taking nuts and washers off we did not know that the bolts were not welded on inside of screen. The weight of the plate caused the bolts to angle downward and the plate to slide off bolts smashing employee's left hand.
While repairing shredder tire, a sliver of metal stabbed employee in the left thumb. Employee removed the metal but a small piece remained embedded in the thumb causing infection and swelling.
2 employees were grinding on a chain conveyor approximately 6' apart in the dome area of the cement plant, one employee felt a piece of metal fly into left eye. EE was unsure if the debris was from EE's own grinding or from the employee next to EE.
Employee experienced pain in right knee while stepping up on a platform. Restricted Duty.
Operator was trying to put #8 heater on the bad condensate line using a wheel wrench on the wheel valve. The wrench slipped smashing ( R) middle finger between the wrench and piping.
On 01/31/18, EE reported that while working in the rail yard ee slipped on ice & twisted left ankle. EE was not wearing ice cleats at time of the event. EE was offered & refused medical treatment. On 02/05/18, a medical evaluation and diagnostics revealed a fracture on left ankle. EE was put on restricted duty as of 02/05/18.
EE was walking between the A track and Runner switch at the Landing at Solvay when EE slipped on a piece of ice falling on buttocks. EE felt a pain in lower neck/mid back area and a tingling in left hand. EE was taken to the ER where EE was advised a follow up with EE's primary physician, restricted to light duty and discharged.
Employee stated was attempting to install a roof bolt on the Unit 1 Section. In the process of the employee drilling a hole to install the bolt a piece of rock fell from the mine roof, slid down the T Bar of the bolt machine, and struck the injured employee on the top of back.
Greasing the loader. Several grease fittings weren't taking grease, needed to be changed. Employee climbed ladder, attempted to open door from too far down. Employee lost grip as they opened the door with the right hand, causing them to lose balance and fall, landing on right hip.
Employee burnt the palm of EE's hand on a vice that was used to make brackets
Employee was assisting with the replacement of an outrigger on the mechanical scaler, when the hydraulic cylinder that was being supported by a block of wood fell from the block striking the employees finger. The employee was seen at a medical center and released that night.
While performing routine maintenance/clean-up near tail pulley of conveyor miner felt a pain in EE's left knee while twisting and bending.
While loading 3/4"" cable into a work boat, the miner lifted several bound wraps of cable over the handrail and let go. When the loose end of the cable went over the rail, it whipped up and struck the miner in the right elbow. This resulted in a contusion on the elbow and tingling in the right hand. The miner returned to work, but with medical restrictions on right arm.
Employee was carrying ANFO bags from the truck to the shot, about 30 feet away from each other. As EE walked up a slight incline to the shot, EE felt a very painful pop in right calf muscle and EE immediately went to the ground. After sitting on the ground for a few minutes, employee got up to see if EE could walk and couldn't bare much pressure on the right leg without pain
While removing a machine guard on the overland belt tail pulley, the miners left hand was pinched between the machine guard and frame. The tail pulley was filled with rock. A rock moved while removing the guard causing the guard to shift and pinch miners hand. The miner suffered a crushing injury/contusion to left hand. No open wound was caused and an x-ray revealed no fracture.
While pulling in a downward motion attempting to open manual overhead garage door, employee felt soreness in back area. Employee was taken to local Urgent Care for observation and diagnosed with a strain of muscle and tendon of back wall of thorax.
The employee was lifting a piece of ductwork improperly. EE was on knees and extending right arm away from body to push up on the piece of ductwork being assembled. In doing this function the employee reports that ee strained a muscle in lower back
Employee was climbing inside of the crusher and hit right knee on the grizzly finger. Employee requested medical attention in May when employee realized the injury wasn't healing. Employee received surgery on August 30 and is on restricted duty.
Maintenance tech moving barrels off of a pallet had foot catch between two slats of a pallet. Upon trying to free foot, the employee felt a pop in knee and it began to swell painfully.
Employee was operating a loader in a stockpile when the bucket struck a large rock and caused EE to whip forward in the cab. EE refused medical treatment at the time but went to be seen 2/7/18 for a left shoulder strain.
Employee got a metallic foreign body in right eye from welding.
1/25/18 employee was walking along the belt line at the crusher and rolled their left ankle. Employee was sent to Concentra to get evaluated. Employee was diagnosed with a muscle strain on the left ankle. Employee was given a prescription and put on light duty. Employee was released to full - regular duty on 2/14/18.
Employee was washing water truck when EE slipped on the stair and hit EE's knee on the step.
On 1/25/18 employee was taken to the company clinic and received stitches for a facial laceration. Employee, while performing clean-up duties, tripped over a 6"" metal channel and fell to the ground. During the fall, the left side of EE's face struck another channel resulting in a laceration on the left cheek.
Employee received scrapes on the back of EE's calves due to ill fitting boots. Moly trioxide came in contact with the scrapes causing a chemical burn
Employee cut the palm of right hand while handling a piece of metal.
EE was disconnecting electrical harness & connector would not release. EE used screwdriver to release locking pin on connector. Connector broke, allowing screwdriver to slip and puncture EE right hand between thumb and index finger. EE was take to clinic.
EE was walking on a trailer, slipped and fell because of the frost.
Employee was sitting on the mine floor at the rear of the roof bolter that ee had been operating. A piece of material fell out from between roof bolts and struck ee on the head, neck and shoulders. EE was examined and released by medical facilities but did not return to normal job until 1/29/18.
While walking up the stairs with a bucket of tools, ee threw out back.
At approximately 3:40pm, EE was dismounting haul truck #718's flexible ladder and missed the last step to the ground. EE was not using 3-point contact at the moment left foot missed the step. EE reacted quickly, and grabbed the handrail with right hand to prevent ee from falling backwards
Employee was walking from one area of the plant to another when EE slipped and twisted knee.
While walking across a flat area of packed snow, employee slipped and fell, resulting in fractures to the scapula.
Employee spent the day walking within the kiln area and received blisters to the right foot, which required medical treatment. A preexisting condition prevented the employee from being aware of the injury. The employee was placed on restricted duty 2/1/18.
Per L&I form completed, employee was walking to work area, slipped on ice patch and hit back of head.
The employee attempted to step on material at the bottom of a rib after the employee completed identifying (spray paint) the rib. Employee fell and bruised back.
EE was walking to vehicle at the end of shift when EE slipped on a patch of ice that formed from that day's melt and had pooled in a small area on the walk. EE was treated by a physician for a sprained left knee, prescribed a pain medication and placed on restricted duty pending the follow-up appointment.
The Maintenance Journeyman A was bent down vacuuming the compressor room and when the Maintenance Journeyman A raised up felt a pain in lower back. The Maintenance Journeyman A did not seek immediate medical attention until 01/26/2018
Employee was changing a bit on a blast hole drill when left hand was struck by a wrench resulting in a laceration and fracture to the left ring finger. The laceration was closed with sutures
EMPLOYEE HAD JUST CLEANED A MATERIAL SPILLAGE INTO BOBCAT BUCKET. EMPLOYEE HAD ALREADY CLIMBED ONTO BOBCAT, BUT AS EE TURNED TO SIT INTO THE CAB OF THE BOBCAT, EE'S FOOT SLIPPED OFF OF STEP AND WEDGED BETWEEN BOBCAT BUCKET AND FRAME OF BOBCAT CAUSING EE'S RIGHT FOOT TO TWIST.
Slipped on ice in Met Ops Parking area and came down on one knee.
Employee walked by scaffolding, rubber flashing fell off overhead work area and struck employee on left arm.
01/19/18 Employee unhooked Alimak hose guard to fill lubricator for pump. EE noticed slack in hose & turned on air to take up slack. Hose tightened causing guard to bounce contacting right cheek resulting in a small puncture. Seen by site medic, referred to ER, restrictions issued keeping employee to light duty.
Employee was walking around the rear of the articulated front end loader, inspecting due to the loader over heating. EE slipped on the snow/ice strain EE's left knee.
EE turned to walk to the mechanics truck 10 feet away, to pick up a tool. As ee stepped over power cords and tripped causing ee to twist/roll and fall. EE immediately reported incident to Supervisor. Injury developed as a contusion on the right foot requiring prescription strength anti-inflammatory.
The Front End Loader Operator on the night shift was leaving the Clay Shed work area to clock out at 7 AM. The Operator left the Clay Shed walking down the inclined driveway past the west side of Fluid Bed. The Operator fell to the ground injuring the left ankle after stepping on a patch of frozen clay.
Employee was walking beside the conveyor in the west tunnel when ee slipped on ice landing on ee right side.
The employee was cutting a stockpile with a front end loader when a large chunk of frozen road gravel fell from the top of the stockpile onto the front of the loader and into the cab. The employee suffered minor cuts on the right hand and lower back pain from removing material from their lap in order to exit loader.
Employee stepped down from a supply trailer and twisted EE's knee
Upon restarting conveyor ee reached around a guard inserting a 4 ft pry bar contacting pulley and belt causing hands to contact conveyor frame. EE was taken to the emergency room due to lack of movement in middle finger, if not better in a few weeks to see a specialist. 2/16/18 saw specialist, referred ee to a hand doctor. 2/21/18 hand doctor performed surgery to fix tendon.
The employee slipped and fell on rough and frozen ground as EE stepped from a telehandler, resulting in a strain to EE's left ankle and shoulder. EE did not seek medical attention until 2-21-2018.
On 1/18/2018 Employee was cleaning the #3 belts in Metcalf Secondary and felt a soreness in the right shoulder area. On 1/19/2018, the employee reported the event to ee supervisor and was taken to the clinic. On 1/22/2018, the employee was returned to the clinic and was given restrictions.
Employee was shoveling gravel and felt slight pull in right shoulder.
The employee was using a knife to check the up and down motion of a sheave at the #2 booster pump. EE was spinning it by hand. The sheave hit the knife and cut the employee's right pointer finger.
Contractor was notified of injury on 18 Jan 2018. Employee was grinding outside and welding on scalper tower floor entire shift on 17 Jan, noticed eye irritation around 2pm. Irritation persisted and employee visited hosp urgent care on 18 Jan. Employee sustained arc flash burn to right eye.
Supervisor was walking to EE's office when EE slipped and fell backward landing on EE's right upper back. Seen at Gulf Coast Occ Med and returned to work after negative x-ray. Reported change in symptoms on 1/29/2018 and was seen by Ortho Dr. on 1/30/2018 and given restrictions and medications.
The employee was reclaiming 50lb bags and strained EE's back.
Drill operator slipped while climbing down boarding ladder striking right knee on ground.
Employee slipped on snow-covered ice during heavy snowfall in the operator's parking area. Employee fell backward onto left shoulder.
Employee was getting a fuel hose to fuel a loader and slipped on ice and snow pack, employee stuck hand out to catch self. Employee's hand landed up against the ladder bending index finger back and fracturing it.
While preparing for end of shift, the employee slipped on a small rock on way from one MCC room to the next causing a left ankle sprain.
Miner was operating a CAT D-350 Articulated Haul Truck, hauling C-4 Dust to Stockpile. Upon returning in Snowstorm to Line 2 Load-out, Right Front Drive wheel slipped off roadway into a drain depression, jarring the driver in cab and causing later back pain.
Employee was trying to loosen a bolt on liner in #2 discharge chute. Employee was positioned sitting on a scaffold plank, EE was tied off above. EE was using left arm to push forward on wrench and hand slipped off the wrench making contact with the wall in the chute causing injury to left index finger.
Employee slipped on ice/snow and fell in roadway between kilns and hydrate building at ground level. Slip and fall resulted in an ankle sprain.
An equipment operator was in a Cat 980H FEL working at the Aglime stockpile. While at the face of the stockpile working the pile a large chunk of frozen material broke free falling about 15 feet & landed in bucket. The weight of the material caused the loader to tip forward causing operator to be thrown forward in the cab making contact with the door glass and cab support pillar.
Employee was shoveling gravel and strained shoulder.
Employee was wearing gloves and getting tools out of a toolbox. The toolbox lid closed on the employees left index finger. Once the employee removed employees glove employee had a small cut that required stitches and a finger splint. The employee was released back to work with light duty restrictions.
ON 01/13/2018, EE was walking to mobile equipment and right leg became stuck in mud up to about shin. EE was offered professional medical treatment and declined. Instead treated leg with ice (first aid). EE requested professional medical treatment and was treated on 02/26/2018. EE was assigned restrictions by Medical.
Employee was guiding a piece of steel plate from a cooler while using a tag line. Employee over-exerted when EE pulled on the line which caused EE to strain a muscle in the chest area on the right side.
Employee Stated that EE was walking from #4 entry to # 6 entry EE was proceeding through a set of fly pads in the number 5 entry while looking at the mine roof EE steeped in a hole and twisted EE's left knee.
Employee slipped on ice while moving rail cars with shuttle wagon. Employee is experiencing pain in right ribs near the kidney.
Employee stated EE was holding a hammer punch while another employee was using the hammer to strike. The employee swinging the hammer missed hitting the frame on a shuttle car and the hammer bounced back striking the other employee in the left forearm
On 1/11 employee slipped and strained knee. Worked two scheduled days was then off two scheduled days. When employee returned to work on 1/16 requested medical was seen and prescribed physical therapy with restrictions.
The miner was cutting belting with a box cutter when the knife slipped and they cut their leg.
While placing liners in the finish mill, mill liner fell from mill and struck employee
Employee injured left finger while setting cribs for side frame rollover
The employee was in the process of knocking out a 12x8 solid block out of stopping when it struck the right arm causing a bruise. The employee later sought medical attention and was placed and restricted duty for seven days. This incident is being reported as a restricted duty.
EE was rebuilding slurry pump at booster EE took rubber guard off of hub seal on pump and laid it on ground next to work area as EE continued to perform task EE tripped on rubber guard and fell on stump and injured lower right side of back and cracked a rib.
Employee was lifting a metal seal when hand slightly twisted causing a wrist sprain.
EE was walking toward the shop, slipped on a rock on the frozen ground, fell to the ground and dislocated a shoulder.
The employee was helping sort out steel beams. The beams were standing on 4x4 boards, when a forklift picked up a beam off of the front of the stack and the rear beams slumped due to a depression in the ground causing the rear beam to become unstable and roll over onto the employees foot.
Employee went to the clinic due to shoulder irritation. EE returned to work on modified duty, the doctor thinks the irritation may have been caused by an incident that occurred in 2011.
An employee placed a green horn on the edge of the bed of the truck to clean crater from it. The green horn shifted and fell from the truck, striking the employee's foot causing a fracture to the 5th digit metatarsal.
Employee was getting a drill core out of the tube and struck finger with a mallet. Employee received 2 stitches.
Employee started to pick up pump suction bowl, felt pain in back.
EE was cleaning mud from around the diverter in the elevator. EE was lying on EE's stomach on the catwalk and reaching down into a small access door. After EE had cleaned out the mud EE started backing away from the elevator while still on EE's stomach. While doing so EE felt a pop in EE's back. EE stood up and had pain in EE's back. EE was diagnosed with a back strain.
Watching employees move a piece of steel - reached up to help and jammed finger against the steel
Employee received restrictions due to sustaining 2nd degree burns from hot material contacting right shin. EE poked material that had plugged a chute and it came out towards ee rather than staying in the chute.
Miner tripped and fell, hitting back of head, causing laceration on back of head that required 7 staples
EE was retrieving square steel tube from surface maintenance shop storage rack. The tube fell, struck EE's left foot causing left first metatarsal fracture, transverse, acute, non-displaced injury. EE returned to work, light duty in office. Follow up appointments are scheduled Jan 9th & 11th.
Employee was boarding a truck and felt a pop behind left calf.
Employee sustained laceration to the left ring finger from contact with a sharp object. Laceration required 5 stitches.
A Utility Swingman was using a sledgehammer to break up the syn-gyp that had frozen along the transition area of the syn-gyp belts. On one of the swings, the employee hit right hand thumb against a part of the transition.

Employee stated was on knees beside the mule washing the cats out when a piece of rock measuring 32""x44"" and 1-4"" thick fell from the mine roof striking employee in the left arm and shoulder.
Employee stepped in a hole walking across the section employee was seen by a doctor and released to full duty. Employee was refereed to a specialist and was scheduled to have surgery on March 5th employee returned to work on restricted duty.
Stored Energy from the metal of an Ore pass Can. AR 450 3/8 inch thick. Repairing damaged can and replacing bad sections. When the Can was cut from top to bottom all the way through, stored energy from the damaged plate came back and struck employee in left hand. Crew also was using man basket to reach location and propped up against to stop bounce back from damage plate.
Individual was performing a pre-shift walk around EE's truck and tripped, injuring EE's elbow. Original doctor visit allowed employee to return to work without restrictions due to no findings on x-rays. Employee returned to the doctor on 1/8/18 due to continued pain . CT scans showed an elbow fracture and placed employee on light duty.
As Employee was descending from the Texas Crushed Stone bulk truck, EE tripped on the last step. EE landed on feet, but in an awkward manner. It caused EE to tweak back putting EE in severe pain and not allowing EE to stand completely upright.
Miner had just driven a split set bolt and was taking off EE's long driver when the driver slid down EE's hand and piece of banding used to hold the driver in place cut through EE's glove, lacerating the palm of EE's hand.
Employee stated that they were taking apart the frame for the storage unit on 20HG. The hauler winch was hooked to one end so they had to pick it up and crib it to unhook the winch rope, while unhooking the rope the coal under the far end of the frame crushed out causing the frame to creel on the crib striking the employee on the right foot.
Concrete crew placing concrete pad with pump truck. Temperatures single digits, wind chill sub-zero digits. Hose line blockage, became loose, started to whip. Hose operator moved out of the way, lost footing, fell onto the rebar mat hitting head. Hose operator sustained laceration above right eyebrow.
EE was hand loading roof-bolt plates into scoop bucket. EE finished loading the plates and went back to the section. EE was exiting the scoop and felt pain in the lower lumbar region. This incident did not become reportable until 01/08/2018 when EE was placed on restricted duty.
Picked up a 50-pound bag of anfo, turned, felt a sharp pain in back.
Employee was setting up a heating device in our wet processing plant area when ee stepped backwards and slipped on a sheet of ice. The employee landed on wrist, causing a closed fracture.
Employee was installing a washer and washer crushed employee's left thumb cutting off the tip of the thumb. Able to return to work with temporary restrictions. No lifting, carrying with left thumb.
Employee was stepping out from crew boat to the dock and slipped and fell in the crew boat. This caused a contusion to right shoulder and hip.
Employee was working with another employee folding up a portable screening plant. IE proceeded to finish folding the plant without the other employee's help. A moving piece of the cat walk opened up hitting IE's left arm.
Injured employee was removing a 2"" pipe plug from a line. The wrench slipped, and the employee fell backwards. EE threw EE's hand out, striking the backside of EE's left ring finger on the corner of an piece of angle iron. I.E. was seen by physician, treated for a broken finger and placed on restricted duty.
Employee was in impact crusher welding - employee was on knees. When EE stood up EE felt pain in lower back and left leg.
EE was using a screw driver to unscrew the light cover on the 6-5 drill and felt pain in their right thumb. EE was evaluated on 1-3-18 with a strained right thumb and put on modified duty.
The employee was operating a skid steer for snow removal. The bucket caught the scale platform and threw employee forward striking the safety bar with employee's chest.
Injured worker was holding an 8"" bolt in left hand while another worker was striking head of bolt with a sledge. During a strike, the sledge missed bolt head and struck the injured workers left hand causing a fracture to the tip of employee's left ring finger.
EE was attempting to remove a fan blade from FA-1301 and was struck in the face with the blade. EE had the fan locked and tagged out. The fan slid off the shaft as EE was removing it and was struck in the face.
An employee was using a loader to do some cleanups, while going into the pile for a bucket, employee went into a dip on the road and the Operator was jolted around in employee's seat and which caused a muscle strain in employee's upper leg.
At the start of a Monday shift, the Quarry Haul Truck driver bumped the berm while backing into the load out bench, causing a jostling to the driver's body. When dumping the load, the driver felt a twinge in lower back when the driver twisted. Doctor stated strain, and advised one week of no truck driving.
Using 10 1/2' burning bar cutting loader bucket tooth out of crusher. When tooth released the pressure pushed the burn bar holder into face causing small laceration on cheek close to right eye
Employee was struck by a piece of metal that broke off of a hand tool while trying to line up a hole on a liner.
EE was reaching across the front end of the scoop ""UL#7"" to access a saw, EE's left knee struck a loose bolt on the machines fender causing a laceration. EE and the supervisor cleaned up the laceration and did first aid, EE later went to the hospital and they closed the laceration with 9 stitches.
Employee was placing chain back into toolbox when toolbox lid fell cutting employee on head
Employee was using a pry bar to break material loose from hopper. Bar slipped and pinched employees finger against the handrail upright.
Weather was cold and the tank opening tends to be stiff and hard to open. While operator was placing their feet to get sample, the lid fell and hand was in the way and the lid fell on their hand.
Congested work area working in a non permit confined space. Limited space for movement. Employees left pinky finger sustained a laceration requiring stitches.
Employee stepped off a skiff onto the trailer of the skiff and lost balance, employee extended hands as they fell forward, they bent the right middle and ring finger backwards causing laceration at the top joint. Employee received sutures on both fingers.
While changing the drill bit on a magnetic drill press, the spindle of the drill press dropped suddenly pushing the drill bit through the pad of the miner's ring finger on right hand puncturing it. Three stitches from a doctor in the local clinic were used to close the wound.
An employee proceeded remove a hose that needed to be changed. The hose was still under accumulated pressure and was not bled out. As soon as the bolts were loosened, the high pressured oil sprayed on the employee who was working on removing the hose resulting in abrasions on the face. On 2/11/2018, the employee was prescribed antibiotics to aid with healing.
While splitting firewood using a wedge and sledge hammer, a piece of the wedge broke off and struck the miner in the leg. Later, it was found that a piece of the wedge had penetrated the skin of the miner's leg. The metal piece was removed by a doctor in the doctor's clinic.
Injured was using a slate bar to pry over a pallet of roof bolts to expose a hole in a bolt tray to put a chain thru in order to pull the tray into a scoop. In the process of prying for the hole in the tray the injured smashed the tip of left middle finger with the slate bar causing a laceration which took 3 sutures to close.
While installing and checking alignment on a dust collector belt, employee received a laceration on right index finger requiring stitches.
Employee was removing the hydraulic tank from a 992K Front End Loader; the pressure in the tank caused the lid to shoot upwards striking the employee in the forehead.
Degas blower not aligned with borehole and attempting to force 8"" hose onto borehole using camlock fittings. Hose slipped out of camlock and struck employee in mouth resulting in a split lip.
EE was performing hot work on a crusher roll, as EE stepped back to review the cut EE had made, the limestone fines found in the crevices of the roll burst outward, causing dust to enter the employee's left eye. Employee was seen by Dr. and a corneal abrasion was found and prescribed eye medication.
EE was moving the hood rake to sweep the floor upon moving the rake, loose dust from above fell down and got in between the safety glasses effecting only the right eye. EE was treated for the injury by a Dr. on 2/1/18 and received a Rx for eye drops to treat the eye.
An apprentice millwright attempted to remove by hand a frozen chunk of bauxite from the feeder. The chunk rolled pinching right ring finger between the chunk and the feeder frame. The wound is a laceration to the right ring finger.
Employee was cleaning an area around #1 dodge coupling with starting fluid. Mechanic was cutting lid off with a torch. Fumes ignited and blew back on employees right hand and right side of face.
EE was working on a roll crusher that was being rebuilt, while attempting to free up a bolt that had been cut from a sizing segment, the pry bar ee was using slipped and struck the employee on the lower right side of the jaw, breaking a wisdom tooth. EE had the tooth extracted by an oral surgeon on 1FEB2018, sutures were used to close the incision.
Employee was attempting to remove a drill steel that was stuck in the mine roof. As the employee went to place another drill steel back in the chuck, the steel that was stuck, fell out striking the side of the employee's left index finger. There was no lack of job skills, training, PPE or equipment protective devices.
While trying to free a stuck drill steel, the steel got in a bind and popped out striking the employee on the right hand. X-rays indicated a hairline fracture.
Employee was getting down a vertical ladder, when EE felt pain in EE's left shoulder and part of EE's left back.
Employee is a Splitter Operator at the Chilton Quarry indoor within the Chilton Manufacturing Facility. Employee was pulling a stone towards self when the stone slipped causing employee to punch finger between two stones. The pinch resulted in a laceration to left pinky finger.
While installing a door on a crusher when the wind blew some crushed rock dust into employee's left eye. EE states that EE was wearing EE's safety glasses.
Employee had placed a bag of rock salt on top of the barrel and took razor knife to cut the bottom of the bag. When ee went to Cut the bag, it slipped off the top of the barrel. EE went to grab the bag with left hand and the knife was in right hand with the blade out, lacerating thumb
EE was trying to remove a railroad tie when left index finger was smashed. The employee was taken to clinic steri-strips and glue were applied to the left index finger and ee was released without restrictions.
Employee was attempting to remove a coupler on a waterline in the 4W beltline, when hitting the coupler the pipe struck EE in the upper lip. The injury resulted in a laceration approxmately one inch long requiring one running suture. Cause of injury was awareness. Compliance with rules/regs,PPE,mining equip/systems not a factor. Miner proficiency may be a factor.
A Quarry Tech was driving an empty haul truck down into the quarry along a haul road. When the front driver's side truck tire hit a ~ 4"" deep depression in the road the truck gave the Quarry Tech a jolt which jarred neck causing a strain.
Employee was taking lunch break and was cutting avocado with personal knife. The knife slipped and contacted left hand ring finger resulting in laceration. Employee was taken to the mine EMT, the wound was cleaned and bandaged. Employee was taken to hospital where employee received five sutures to laceration
Employee was sorting 1 1/4"" steel, mill balls used to grind cement in a roller mill when a stray mill ball rolled down the chute incline and ramped up towards the employee who was in the process of securing the chute discharge to the upright position. As such, the steel mill ball struck the employee in the mouth.
Employee was pulling the guard out of a conveyor with another employee for maintenance. The weight of the guard shifted pinching the employee's left thumb between the guard and conveyor. This resulted in a cut or tear of the skin next to the thumb nail. Required stitches. There was no damage to the thumb nail. Employee was wearing gloves at the time of the incident.
Employee was getting ready to drill a roof bolt hole in the mine roof. A piece of rock measuring approximately 1'L x 1'W x 3""T fell from the mine roof when the drill steel touched the mine roof. This resulted in a laceration to EE's right forearm and took 4 staples to close the wound.
Manufactures Instructions for pump rebuild did not specify presence of high voltage connecting socket on board. When plugging connector back into board the workers finger made contact with the screw on the connector that carried the Hot power. The workers right arm on the analyzer and the current went through them. No pain after incident only muscle spasms afterward.
Dry conditions at the face. Employee was building a return wall. Block fell off of top row and hit finger resting on top of another block.
Employee was sliding the Knuckle boom extinction back in and pinched right ring finger and received 4 sutures.

Temporary worker attempted to lift a case & stack it on a pallet when EE felt a sharp pain in EE's back. Diagnosis: lower thoracic pain & spasm. Restrictions of may lift up to 10 lbs, no twisting & bending.
Employee was hand scaling mine roof at face. When hand scaling, a small chunk of salt fell and struck employee in mouth. Injury caused a loss of a tooth.
Employee was removing a bolt from a flanged valve and a small piece of metal became embedded in left hand.
Employee lost balance after bending over to remove a wheel chock. Fell backwards onto back. Employee was transported to the ER via ambulance and released the same day. (Per a MSHA accident investigation, this is not a reportable accident.)
Miner tripped and fell, striking head on a handrail.
EE was standing behind the employee trying to remove a worn tooth on the loader bucket. A small piece of the tooth came off hitting EE in the left cheek area below the safety glasses.
4900-121 Shop, move full 55 gallon drum of oil off timber truck, barrel slid causing left little finger to be fractured between two barrels.

As EE stepped to the back of the CM to help a coworker, EE turned back to the head and struck the back of left hand on a sharp piece of metal causing a laceration. EE was transported to ER and received 6 stitches. EE returned to work on next scheduled shift.
While installing a water pipe, EE accidently opened an air valve causing an injection injury
EE was carrying a safety can and slipped on ice.
An electrician was investigating why the right side canopy on roof bolter would not lower; when the canopy dropped rapidly without warning. The canopy mast assembly did not drop when the hydraulic cylinder was lowered; it became stuck and then dropped. The canopy struck employee on the head causing laceration that required 3 sutures to close.
Employee was pulling a wash down water hose up to the next floor of the prep plant by hand. The water hose slipped through right hand and a metal hose clamp pulled through their hand and cut the inside of the hand. Employee took themselves to the hospital and received 8 stitches. Employee returned to work that same evening and has missed no work.
Maintenance was being performed on A 9"" long bolt flew through the air and hit the face.
The employee was performing the duties of sanding the railroad tracks to aid traction for both the loader and rail car traffic in the area. The employee slipped on the packed snow while walking and attempted to arrest the fall with their right hand, feeling a sharp pain. an x-ray confirmed that the right wrist had been fractured
Employee, returning from medical treatment without notifying company that EE had been injured on a previous date informed company that EE had a bulged disk in EE's back. The employee stated after leaving work EE slipped. The employee said EE didn't fall down or hurt self. After waking up EE felt pain in one of EE's legs.
A rock measuring 10"" x 8"" x 1"" to 1/2"" fell from the mine roof and struck the employee in the right cheek and upper lip causing lacerations to both the cheek and lip.
In verifying operation of air actuated valve, employee placed hand on or near the valve seat and valve closed, pinching the middle finger of the right hand. Doctor diagnosed an open wound of right middle finger with damage to nail. Treatment included subcutaneous tissue repaired with 2 sutures and an injection of Ketorolac. Employee discharged as fit for duty without restrictions.
EE was riding in the passenger side of a water truck in the Quarry, when the water truck tipped over. EE is a temp employee from Express Temp Agency.
I was cutting a ""C"" channel while in a man basket. At the end of the cut the material folded and caused the grinder wheel to backlash and cut my finger. My helper witnessed the cut. I saw the laceration and knew I needed stitches. I drove myself to the emergency room and received three stitches on my right hand index finger.
While un-spooling wire rope, employee was walking backwards and tripped over test weights.

Employees were disassembling a dozer assembly. An employee went to toss the sling that had a 16 mm shackle attached to it. The pin came out of the shackle and struck the EE in the mouth, chipping a tooth.
Employee was cutting belt when employee slipped and cut left wrist with utility knife. Employee received 3 stitches.
Employee was running a communication line from second floor to seventh floor. While performing this task, employee got lime in right eye. First Aid was administered and eye was flushed, then later in the shift employee said eye was still irritated. Taken to the doctor where the eye was examined and flushed again. Employee was prescribed anti-biotic as a precaution.
On 18 Jan 18, injured employee (IE) strained left knee while moving 40' of vac truck hose. IE felt slight pain, reported the incident to supervisor, and started first-aid/self-care treatment. On 29 Jan 18, IE wanted to go to the doctor because IE felt the knee was unstable. The doctor prescribed six physical therapy treatments and the IE is feeling better.
EE removing transmission on pickup when tool slipped causing EE to hit hand on transmission or framework of truck cutting top right pinky finger. At TOI, EE refused medical treatment, just bandaged cut. EE contacted supervisor at 9:42PM and went to hospital resulting in 3 stitches, shots and a splint.
Employee was using a hammer to break up some material when the hammer handle failed causing the hammer head to come back and strike the employee in the face. The impact resulted in a laceration to the lip that required sutures.
Employee was performing an overhead patch repair on the kiln eyebrow. EE was tamping plastic using a hand held pneumatic pounder. The air exhaust from the tamper blew material behind safety glasses into right eye.

While using a pry bar to scale down some loose draw rock a piece rolled back causing a laceration to employee's left thumb. (3) sutures were required to close the wound.
Miner was on a step ladder, approximately 5' High. Miner had 3 points of contact while attaching a hose. Miner twisted while installing the hose shifting EE's body weight causing the ladder to kick out from under EE. The miner fell striking EE's rib area. Miner was assessed and had no signs of injury but was taken to emergency room to be checked out.
Employee was changing motor and dropped it on finger, and smashed ring finger on left hand.
Recovering high voltage cable from 4E #2 left entry between 2 and 3 block. When EE unhooked the cable hanger, the hanger straightened from the weight of cable and drove the closure hook into EE's right index finger, palm side, between the 2nd and 3rd joint, resulting in a puncture/avulsion that required minor debridement and 3 sutures to close.
During a mid-shift inspection of the work area, a hazard was noticed. The employee thought it to be a good idea to look further into the hazard to see if EE could fix it. Watching the kiln and assessing danger EE moved closer to the Gaurd to see if it needed removal or adjustment. EE's hand rested on the guard momentarily and the guard failed pinching EE's finger.
Employee was opening a railcar lid when they smashed a finger between the lid and a loading spout.
Employee was carrying a piece of I beam and slipped allowing the beam to fall on left index finger.
EE was attempting to throw a crib block thru a door when EE struck EE's right thumb between the door frame and the crib block. EE suffered a laceration that required eight sutures and a fracture of the tip of EE's right thumb
Employee has a personal medical condition which caused EE to feel light headed and fall while walking up BC2024 catwalk. Employee fell and struck the back of head on the catwalk resulting in stitches to close a cut on the back of head.
Employee was using a forklift and noticed that there was slack in one of the chains which raise and lower the forks and decided to adjust the chains. The Operator tried to realign the chain onto the pulley, when the chain moved and caught the right hand between the chain and the pulley. This resulted in avulsion to the right index finger
Employee sustained a miner laceration to the back of left glove index finger requiring stitches to close the wound.
Employee was roof bolting on the #11 unit left bolter installing a 5 foot fully grouted roof bolt when the left middle finger glove got caught in the pin causing a laceration to EE's finger requiring stitches.
Miner was using a 30 ton ram and a 16 lb hammer to replace pins on a D10 undercarriage. While swinging the hammer to loosen the pins, ee felt some pain in back. EE reported this to employer, but didn't want medical care. Unbeknownst to the operator, employee sought chiropractic care. On 2/20/18 employee asked the operator to provide medical care and filed a claim.
Slipped on ice hurting left knee. Disobeyed supervisor regarding situation of returning to work.
While operating the longwall shearer, there was a small coal outburst and small coal particles hit employee in the face and left eye. Injured employees eyes were flushed and EE finished EE's shift. Employee followed up with a doctor who removed a small piece of coal from EE's eye. Employee was wearing safety glasses and has proper training. No rules or regulations were violated.
While grading the road with a loader, employee struck the edge of a concrete pad, causing the loader to stop. Employee hit head against something resulting in a cut on the right side of head requiring stitches.
The employee was using a poking bar to poke the frozen chunks down in the feeder, on a forward stroke the employee caught the ring finger on left hand on the middle bar of the hand rail pinching finger between the poking bar and the handrail. Employee received two stitches.
Employee was installing an engine(8000lbs)in a 988K using a telehandler. The Employee was standing on the 988 guiding the engine in place when ee became off balanced (salt on standing surface), ee reached out to stabilize self when ee caught the back of left hand on the engine saddle and cut it. EE was wearing cut level 2 ANSI gloves.
Employee parked truck on the scales and when ee went to exit the truck, ee slipped on the ice and hurt shoulder and back. The employee did not seek medical attention until 01/23/2018. This injury became reportable because ee was given a physical therapy prescription.
While unclogging a chemical supply line material inadvertently contacted miners left eye. Causing a chemical burn to the eye
