Accident type, without injuries

At approx. 11:20am on 11/30/17, a fire was discovered at the primary MCC room. Fire department was dispatched and arrived at 11:30am. Due to fire involving switchgear & located next to 4160v transformer, firefighting activities were delayed until power could be de-energized. The fire was extinguished at 12:38pm, minutes after electrical services could arrive and disconnect power.

The freight elevator at the roaster was commanded to travel from the third floor to the twelve floor. The elevator traveled approximately 15 feet in the opposite direction toward the ground floor and stopped in between levels. There was an employee on the elevator during this incident. The employee remained on the elevator for 2.25hrs before being removed.

Roof fall in the # 9 room of the C butt that measured 20 ft. wide x 70 ft. long above anchorage. A visible geological feature was identified in the adjacent entries contributing to this fall.

The elevator was taken out of service at 11:45AM. Schindler Elevator Co. came to inspect the elevator and stated that the rope gripper pads are not worn in and the rope gripper trigger was broken and also the governor trip latch is bent or worn. Lake Shore came and inspected the elevator and did not find anything wrong with it.

A roof fall was discovered during weekly examination on November 27, 2017. The fall is located in the Main North section, crosscut #14 between entries #3 and #4. Approximate dimensions are 12 to 22 feet long, 16 feet wide and 6 feet high. The area was bolted with 6 foot resin bolts. The area was developed March, 2005.

A roof fall occurred in the underground mine at location of 33 and 2v, the fall blocks passage thru this area. Operators were approximately 1300ft away from the area of the fall when it occurred. No miners were injured.

An unplanned roof fall occurred in a regularly traveled area above the anchorage zone in the 3B6 Panel at cross cut #12, entry #5.

Entrapment of 1 person due to doors on elevator not being firmly closed causing door switch to make up intermittently and stopping elevator. No other 50.20 criteria applies.

#3 hoist load cells lost calibration during test trip that started at 5:30am. Troubleshooting began at 5:56am. GL Tiley was contacted via phone while third shift was evacuating the mine with the emergency hoist. Load cells were re-calibrated at the right values and another test trip was performed with success. First shift went down at 8:05am.

An unintentional roof fall has occurred on the S4D travelway. The fall was approximately 35' by 10' wide and approximately 7' high. An action plan to rehabilitate the area was submitted and implemented for the area.

A roof fall was discovered, without injury, at break #106 on the 10 bolt, approx. 19' wide x 30' long x 5' 8'thick. Appears to be a rider seam 6"" above fall. Area had been supported by 5'resin grouted bolts on pattern.

Smoke was discovered on the west high wall in the 5 1/2 West entry. The smoke was not extinguished in 30 minutes. No miners or mine equipment were at risk during this event.

Production Hoist braking system would not release. The Hoist was deemed out of service, evac was called, and MSHA was notified.

A fire was discovered in the office change room by dispatcher at approximately 9:00 PM Saturday Nov. 18 2017. The location of fire was in storage room where the IWT tracking and communications components were stored. The local fire department extinguished fire when they arrived. The fire remains under investigations by MSHA, State and company officials.

A roof fall occurred in the Cedar Grove Mains track entry, #104 XC, that was approximately 45 feet long extending into the X-cut, 20 feet wide and 10 feet high.

There was a clay vein observed in the intersection that is believed to have contributed to the fall in the affected area of B-Mains (5 Belt).

The 1958 East stope was shot end of shift, the night shift miner began workplace exam to wet down muck piles post blast, noticed a cracked shotcrete within the 1958 E/W intersection; immediately retreating to equipment parked in the muck bay approx. 300 feet away from the referenced intersection. The miner contacted the supervisor, soon after the fall of ground occurred.

At 1:20pm EE was operating a Caterpillar 345C excavator when EE noticed black smoke. EE immediately exited the machine and seen large amounts of oil on the ground. This supports the idea that a major hydraulic hose blew causing the fire. EE and other company personnel attempted to extinguish the fire but were unsuccessful.

An inspection door to the Fly Ash silo chute was opened for inspection. When the inspection door was opened; material began to rush out of previously empty chute dispersing material causing limited visibility and respiratory distress.

No. 3 Hoist - hoisting muck when hoistman saw flash by 1750 HP motor, Hoistman shut hoist down; bar holding armatures came loose, tightened, run checks up and running at 1405 hrs.

At about 10:14 am, the hoist tripped out due to a power bump. This also caused a communication error between the hoist drive and the main programmable logic controller. No one was in the shaft when the power bump occurred.

Smoke was discovered on the west high wall in the 6 West entry. The smoke was not extinguished in 30 minutes. No miners or mine equipment were at risk during this event.

The production hoist went down and could not be reset. Troubleshooting began and a fault in the #2 drive was identified. The fault was corrected, test runs were performed, and the hoist was put back into operation.

The elevator at Patterson Creek portal was out of service due to a faulty bottom landing door switch. The switch was replaced and the elevator returned to service.

CAT 777D operator was moving waste to the waste dump at mack's peak. The operator dumped load and started to drive away with the bed in the up position. The operator traveled 340 feet before making contact with a 12kv overhead power line. The power line made contact with the raised bed causing the line to arc and pole to break. No injuries occurred during this incident.

Two persons were entrapped on the Sparta elevator when while riding in the elevator it stopped approximately 42' from the top landing due to the bottom hallway door latch being out of adjustment. The fault was reset on the elevator and it was called to the top where the persons exited the elevator and the door latch arm was adjusted and the elevator was tested.

A roof fall was discovered in the Cedar Grove Mains Intake, #2 Entry, #34 XC. This is not an intake escapeway and ventilation was not disrupted or affected in any way. All approaches to the fall have been dangered off and all persons affected notified.

At approx. 1:15 pm our Old silo elevator stopped with one miner inside. This miner was later rescued unharmed.

There was a unplanned roof fall at the #9 1st South Panel, spade 15+80 #7 Entry.

The fall is located on the 4 west belt at the number 2 break at spad #4114 the fall is approximately 35 feet long 20 feet wide and 5 to 6 inches thick a plan for cleanup has been submitted to MSHA and has been approved.

An unplanned roof fall measuring 12 ft. long X 8 ft. wide X 6 ft. in height occurred at the anchorage point in the #3 belt entry of the 9 Right longwall panel at spad 7+40.

A non injury roof fall occurred at approx. 9:25pm on 11/3/17. The roof fall is located in the No.4 entry intersection at x-cut No.8 on the 1 east mains. The roof fall is approx. 20' feet long x 20' feet wide x 8' feet to 10' feet thick. The roof fall is confined to the intersection at x-cut No.8 The area has been flagged and dangered off to prevent access to this area.

While dropping a rail car down the shaft the crane on the service hoist went down due to electrical issues in which prevented the hoist from being able to be used. All employees were evacuated in a timely manner, there were no injuries, and both agencies were notified.

Loading rock with a 992G loader when the loader apparently struck an undetonated primer. The concussion caused an air blast that shattered two windows in the loader and the back window of a 777F truck. There were no injuries to miners.

Plant Foreman was contacted after hours and informed that a transformer had caught fire on the backside of mine property. Local Fire Dept. had already been notified. Cause of fire is believed to be transformer damage due to recent storms.

Unloading a tightly packed shipping container filled with machining equipment. Stored energy due to the amount of equipment in container and movement equipment inside during travel of container. Mashed fingers between two metal boxes.

While trying to lay down Drisco pipe, the end piece slid on dirt and bumped into the employee. It knocked employee down and in the fall, employee sprained knee and ankle.

EE was assisting with the removal of a broken top belt roller. The roller was under tension from the top belt and when it was pulled free of the tension, it rotated upward striking EE on the face in the left cheek area.

Employee was walking down the slope belt and pulled employees groin area when employee slipped and fell.

Fall onto or against objects

An employee was descending the cab access ladder of a haul truck. The light illuminating the ladder timed out and went dark. The employee continued to descend the ladder in the darkness. The employee thought ee was on the bottom rung of the ladder and stepped down, causing ee to fall and strike knee on the bottom rung of the ladder

Reported while shoveling rock when EE twisted to EE's right EE felt a strain in EE's lower back. Lower back strain

Caught in, under or between a moving and a stationary object

Operator was restacking pallets, when operator backed up forklift, operator did not have left foot completely inside the forklift, which became pinned between the forklift and a concrete pillar. Operator was given basic first aid until an ambulance arrived. Operator received one stitch, and is referred to a foot specialist on 12/4/17.

The miner was climbing down the steps of the loader towards the ground, and upon their right foot making contact with the ground, the miner had stepped on a rock and sprained the right ankle.

Over-exertion in lifting objects

Injured employee was lifting the continuous miner trailing cable when they felt pain and a burning sensation in the stomach area. Injured employee didn't violate any rules/regulations and has proper training. Wearing appropriate PPE. Injury caused by overexertion due to lifting.

Over-exertion in lifting objects

Injured employee was lifting a belt roller into a backhoe bucket and felt a ""pop"" in lower back. Employee was standing on uneven terrain and overexerted back while lifting. Employee didn't violate any rules or regulations and has proper training and was wearing required PPE.

Employee was getting into service truck and the step which is the battery box lid came loose and caused the employee to slip. Slip resulted in a sprained knee.

EE was taking measurements when EE felt a ""pop"" in EE's left hip. EE was brought to the safety building and requested to be taken in. EE was seen at the ER and referred to Orthopedic Doc. EE was seen by Orthopedic Doc same day and placed on restrictions.

A fully trained miner injured left ankle when ee was attempting to change a hose on an air pump. The employee slipped backwards and felt a pop in ankle. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was working on slope belt when some wet material come off the belt knocking employee down. Employee did not start missing time from this injury until 12/4/2017.

Over-exertion in lifting objects

Employee was moving continuous miner trailing cable out of the roadway when ee felt pain in lower back resulting in a sprain/strain injury and lost work days.

Contact with hot objects or substances

Operator received burns to left hand as a result of a fire near the cab of the Caterpillar 770 water truck.

EE bent over to pick up miner cable and waterline to hang it. EE states EE felt pop in lower back. EE continued to work until January 8, 2018. EE was diagnosed with disc desiccation.

Employee was getting out of shuttle car when they hyperextended the left knee. Employee started missing work 12/12/2017.

EE was the MN2 take-up shoveling and washing with a hose. EE was shoveling a large amount of gob and when lifting it to the belt, felt a sharp pain to lower back. EE continued to shovel and stated aggravated back even more.

Fall from machine

Individual was climbing up the ladder of EE's loader to get in the cab. EE had both hands on the rails, but lost EE's footing and fell to the ground. EE hurt EE's right heel and right shoulder.

Struck against stationary object

While hanging ventilation curtain, employee stepped on a cement block, under the curtain, causing ee to twist ankle and experienced pain in the right ankle. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve attitude, job proficiency, or machinery.

Struck by rolling or sliding object

The individual was shoveling under the belt, with their back to the rib, when approximately 1' to 3' of rib rolled out. This covered their feet and lower back.

Employee strained lower back lifting a turbo onto an engine manifold. Note: Employee received initial medical treatment on 12/07/2017.

Employee was carrying a half bag of dust as they were rock dusting around section power center. As they stepped over chain at power center they felt a pop in left knee.

The employee was reaching for a 6' bolt from the bolt tray when the right ring finger was struck by a falling rock. The following were not a factor: Rules, regs, mining equipment or systems, skills, proficiency, training or protective devices. Attitude may have been a factor.

Contractor employee was pulling a rubber tire supply car with a scoop. To avoid hitting a dewatering pump ee decided to set the scoop over against the right rib. As ee proceeded to tram the scoop, the back of the scoop deck undercut the rib causing some rib rock 30"" x 14"" and approximately 5"" in thickness to dislodge striking ee on the back of ee's hard hat and shoulders.

Flash burns (welding)

Possibly exposed to arc welding from fellow worker; working on the opposite side of blower housing. Employee was wearing welding/cutting goggles.

Struck by falling object

A rock fell out the roof striking employee's right forearm, resulting a laceration were employee received eight stitches.

Absorption of radiations, caustics, toxic and noxious substances

A hydrate bag fell off the end of the hydrate bag conveyor belt and busted when the bag hit a corner of a wood pallet. The bag busted causing hydrate lime to go into the employee's eyes. It went into the right eye more than the left eye. It caused eye irritation for which the employee was prescribed ointment and eye drops.

Replacing rub rail on continuous miner. Employee bent over to pick up rub rail and pulled lower back. Employee took self to the hospital and has not returned to work.

Employee stated employee was cleaning under the TSK lime belt when employee felt and heard a pop in employee's right knee. The employee was sent to a doctor, then a specialist and is schedule for surgery.

A fully trained miner walked into a longwall shield and strained their neck and right shoulder. The incident was not reported to mine management until 11/27/17 at which time the employee decided to seek medical treatment. The incident was not the result of any compliance issues nor were any protective guards involved.

Struck by rolling or sliding object

Employee was stocking tray on Roof bolter 157, operator side, getting ready to roof bolt the second cut in No. 4 entry when the rib rolled hitting employee causing injury resulting in fractures to arm, leg and pelvis.

Over-exertion in pulling or pushing objects

The Section Mechanic was prying on miner waterline that was wrapped around the feeder crusher drum when the Mechanic noticed a soreness in left arm. Continued to work regular job at full duty until 12/06/2017.

Struck by powered moving object

IE was repairing bad splice on shuttle car in #5 entry. EE left for 10-15 min, returned & began working on splice. EE had not seen scoop in the area. With cap light & blinking light on, EE was on knees looking down. Scoop carrying supplies in elevated bucket was heading inby in #5 entry at walking speed. Scoop operator rang bell before & while passing through the airlock curtain.

A fully trained employee has no exact memory of anything left knee began hurting and as the shift progressed the pain increased. The alleged injury was not the result of any compliance issues nor were any protective guards involved. There is no known cause of the injury.

The employee was helping roof bolt an outby area when a rock fell and struck the employee's hand. The following were not a factor: rules, regs, mining equipment, protective devices, clothing, skills, proficiency or training. Attitude may have been a factor.

Over-exertion in lifting objects

Employee, from a standing position, reached for backpack, felt tightness and pain in lower back. Employee refused to be seen by an ERT member, requested to leave work. Employee drove self to the ER and was taken off work until 11/24/17.

While performing maintenance on the wash plant, employee aggravated their right shoulder. ***1st day of lost time 2/21/2018 (date of shoulder surgery)***

The employee was pushing a roof bolt up in the hole with the bolter and a 1' wrench and had the tip of a finger between the head of the bolt and the wrench pinching the finger.

A fully trained miner was installing a roof strap when a rock fell from the roof striking EE in the upper lip resulting in stitches to the lip. The accident was not the result of any compliance issues nor were any protective guards involved.

While checking fire extinguishers at Argos Plant, employee slipped down on a flat, muddy area covered by gravel. Employee's left leg slipped forward and turned out. Knee slipped back and ee fell on right shoulder. EE has a bruised shoulder and a swollen knee.

Employee alleges ee was replacing bolts and left thumb went under the bolts pinching ee's left thumb.

Was exiting loader climbing down ladder and landed on twisted ankle.

EE was operating a shuttle car when they hit a rut in the roadway and jarred their lower back.

EE fell off uke bumper while entering the truck. EE injured right wrist, elbow and ribs.

While pressure washing crusher box at Wash plant EE accidentally ran the pressure washer across right foot and the pressure punctured behind the steel toe of boot and left an approximately 1/2 cut on right foot.

EE was tightening a nut on a D3 EE was operating with pliers, both hands using full grip. EE said EE's chest popped & EE let go & drove ride outside & reported to another EE. EE did not see a M.D. until 7 days later, 11/27/17. EE missed 1 day of work, 11/28/17 and returned to EE's normal job 11/29/17.

Struck by falling object

Injured was the rib bolter on the right side of the full face Alpine miner in the face of the future belt entry in St Leo when some head coal fell out from between the roof straps (approx. 4' long, 5' wide and 2-3"" thick) and struck the injured's hard hat, driving it down and causing minor lacerations (no sutures) to nose and lip, as well as a minor contusion behind right ear.

Rock was being dumped approximately 500 feet away from Pit. A rock rolled away and over the berm hitting the drill that employee was operating.

IE was attempting to remove mud from the transmission housing of a pickup truck. IE violated Co policy by not chocking the wheels, not locking/tagging, and not de-energizing the truck prior to maintenance. Resulted in truck moving while IE was still on ground with the tire of the truck contacting ee resulting in a broken clavicle and a laceration to ear

A water truck driver was attempting to turn on the water supply at the beginning of the day. The valve was a properly installed lever-type. Temperature was between 8 and 10 degrees F. The 3/4 inch bolt on the lever connect the valve snapped. The operator fell to the ground breaking operator's wrist.

The employee was on top of a railcar closing hatch covers and then proceeded to walk down a ramp and struck head on an overhead I-beam. The employee was wearing a hard hat at the time of impact.

Temporary employee was assisting with mill overall using an air impact tool and bent over to loosen the nuts on the plow. As temporary employee began to straighten up EE felt pain in EE's lower back. Temporary employee is being treated for lower back pain by medical professional.

When a 992G loader was cleaning the base of a highwall. The bucket of loader knocked a section of rock lose that fell onto the bucket. The loader was jolted around causing the employee to hit their right side on the arm rest. Seat belt was being used.

EE was operating a walk-through bolter, installing roof bolts in the #2L face. During the process of drilling the hole, the drill steel became stopped up with dust and EE did not let off of the boom/rotation pressure quickly enough, resulting in the steel bending and striking EE in the left side of the face. EE began missing time on 11-21-17.

EE pulled slick lines and hooked them up to a pump able crib, turned around and tripped over the slick lines and tried to catch self dislocating left shoulder.

The individual was setting a steel beam on a structure used to place the beam across the conveyor belt when hand hit the waterline and the beam slipped out. This smashing left thumb. Received 12 sutures.

Employee slipped and fell on the driver's side excavator track while cleaning the windshield. While falling, they grabbed onto the grab bar and hit the right lower back on the excavator track. Injury consists of overextension of lower right back muscle, resulting from holding onto grab bar while falling and/or impact with track.

Employee was found laying on the ground in front of the feeder by another employee. Employee stated employee had fallen and twisted employee's back. Incident resulted in a sprain/strain and lost work days.

Injured employee was helping to load an Irwin Diesel Locomotive wheel onto a dolly when ee caught left ring finger between the wheel and the edge of the dolly. The injury caused a fracture to the tip of finger and a laceration that required 4 sutures.

Mechanic was disembarking off of the fuel truck. Misjudged the step and fell backward, hitting their head on the open door, edge of the service truck.

Two employees were carrying a rack wheel onto the face of the E-29 LW section when the rack wheel moved causing the weight of the wheel to shift towards the injured employee which in turn caused the IE to strain lower back.

EE was jostled around in the seat during coal loading, as EE was reaching behind for a load log. EE was evaluated and released to return to duty on that date. On 11/28/2017 EE returned for additional medical evaluation and was held out of service to receive treatment for lower back, making this a reportable event on that date.

Miner tripped on a rock while shoveling. Miner landed on rock, striking lower back area. Miner could move on miners own, but was in pain and taken to urgent care. Currently unable to work until follow up appointment.

Struck by powered moving object

The right side pinner was moving from #7 to #8 entry. The Ram Car was setting idle with the pump motor on waiting for the other car to come out of #9 entry. Employee was moving cable out of the way for the pinner to pull into face of #8. EE was standing behind ram car when the car released brake causing ee to rollback striking employee in back of knee.

Employee was getting out of ee personal truck in the Sugar Run parking lot upon arriving to work and twisted left ankle when stepping onto the ground resulting in a strain to the soft tissue of left ankle.

Battery end operator side tire was coming apart, tramming car to repairman & a flapper tore fender off & came in deck, hit left arm in bicep area.

EE was climbing up the ladder on the haul truck. EE's right foot was on the bottom rung which swung under the bumper. EE's right shin and knee hit the face of the bumper, pushing EE's foot off the step. EE's foot hit the ground, all of EE's body weight landed on EE's right foot. EE was taken to the doctor and being treated for a spraining EE's right ankle

The individual climbed onto the stacking conveyor to lube a few rollers. EE did not lock out the conveyor nor did EE use fall protection. The conveyor was started once EE climbed onto the conveyor resulting in a fall.

Hauler driver of semi-dump unit dumping limestone onto ground at processing plant. Semi-dump unit rolled onto its side.

A contract employee had walked into the shower and had been in there approximately 3 - 4 minutes. employee just finished washing hair, turned around to grab body wash and as they turned, they slipped, their feet came out from under them causing them to land on their head causing a laceration that took 10 staples to close the wound.

Employee was setting prop against the rib in No. 1 entry on No. 2 Longwall, when the rib rolled out onto employee's leg causing fracture to left upper leg.

IE worked without restrictions and was referred to an orthopedic specialist who ordered an MRI. The results of which indicated a torn medial meniscus which was repaired on 03/02/2018.

The employee opened a gate for a contractor to leave. The contractors trailer became entangled with one side of the gate causing the trailer to jump sideways striking the employee who was holding the other side of the gate.

IE stated that the scoop operator had cleaned entry # 7 and part of the line curtain had been torn down. IE went up to help the scoop operator hang curtain back up. While IE was hanging the curtain a rock fell out between the bolts striking the IE on the hard hat.

IE stated that while walking down the travelway of the 1st Main West they stepped in some dust that had a rock underneath causing them to roll an ankle. EE was diagnosed with a sprained ankle. Employee had completed repairs on haul truck. EE was in the process of removing a 150 ton rated jack and cribbing out from under the truck. In the process EE lost control of the jack and pinched finger. This resulted in a laceration that required sutures.

Miner was engaged in maintenance activities with the witness. Witness had handed employee a rubber rail which EE threw down. EE then grabbed a loose metal rail which shifted and the witness heard a thud. The witness looked onto the catwalk and saw the injured employee laying on the catwalk with hand on the back of head but did not see how the injury was incurred.

EE was ascending stairs at an overcast when they stumbled near the top and fell striking the left upper arm on the top of the handrail. This resulted in pain to the lower bicep area. EE continued to work regular shifts through 11/19/2017.

A fully trained miner was injured when miner was putting oil in the continuous miner. While holding the lever for the miner, a rock fell from between the straps striking miner's left shoulder and neck. The accident was not the result of any compliance issues nor were any protective guards involved.

Employee was unloading 42"" crib block from a supply trailer. While pushing the block away from them toward another employee, they felt pain/pop in the right bicep. Employee continued to work conducting normal occupation without restrictions. On 12-12-17 employee was placed on restrictions that prevented them from performing normal occupation.

While preparing the bulk truck to load a floor shot, the IP stood below the auger boom and attempted to connect a flexible loading hose to the auger boom. The loading hose uses a 4"" camlock as the connector. As the IP attempted to make the connection IP lost control and the hose fell with the camlock striking the IP in the mouth causing damage to lip and teeth.

Contact with electrical current

Incorrect components in the VF Drive led to overheating of a transformer and a normally open contractor. Overheating of contractor led to the welding of the internal contacts, resulting in abnormal current flow to area where incident occurred.

Employee was exiting loader, when ee reached the 3rd rung hands slipped on handrails causing ee to fall to ground. At the time it was raining hard and handrails were wet. Took employee to ER to get checked out and they found nothing other then some bruising. Dr's excuse said to return in 2 days but employee was not scheduled to work except for half a day the following day.

employee on street sweeper was on side of road facing east on SR4030 (Gilberton Road). Blue Mercedes behind was almost stopped. Tan buick approached & hit into blue mercedes and pushed it into street sweeper. Local ambulance, paramedic's & police were on site.

Employee was walking inby in the #2 entry on the 9O CM Unit when the continuous miner cable and waterline that was hanging across the intersection was pulled down and struck the employee in the right side of the neck and back.

While taking rib straps off of miner employee felt a pop and pain in right shoulder. This did not involve any compliance issues. Personal protective equipment was being worn and this did not involve job proficiency attitude or machinery.

Maintenance employee was swinging a 10lb maul when they felt a pain in the (R) shoulder. Didn't report until day after injury. Negative x-ray was completed on 12-15-2017 at Gulf Coast remained problematic. Ortho MRI on 01/30/2018 revealed bicep tear and employee was restricted on 2/06/18.

Injured was adding oil to the continuous miner in the 5E section. EE had cut a larger hole in the top of the oil can with a screwdriver and a hammer. EE dropped the hose plug into the oil can and when retrieving the plug they caught their gloved right thumb on a jagged edge of the oil can, resulting in a laceration that required 15 sutures to close.

Employee was cleaning a conveyor belt's transition bellow the conveyor pushing accumulated material with a bar, the bar was pushed by the material causing it to slip, the employee tried to grab the bar and it pinched left middle finger with a ladder causing a cut.

While unloading steel beams from a scoop bucket, employee dropped the beam pinching their fingers between the dropped beam and a beam laying on the ground.

As the employee was trying to fit an oil seal ring (1/2 ring) onto the Ball Mill feed-end swivel bearing, it slipped from ee's grasp and fell approximately 24 inches landing on ee's left foot. The 1/2 ring weighs approximately 40lbs. The ring fractured ee's left big toe. The employee was wearing hard toe boots but the ring fell in just behind the hard toe.

A piece of wire off the panel went through boot and into the arch of foot. Employee was met at the hospital by Strata Safety Supervisor.

Employee was removing a scaling bar that was trapped in the apron feeder. When ee removed the scaling bar, the ladder sprang back to the feeder and trapped ee's fingers behind the top rung and apron feeder wall.

Employee stated that as ee stepped off the platform on the RB machine ee slipped and fell. This caused a pain in back. Employee was taken off work on 11/8/17 on a follow-up visit to the doctor.

Employee was clearing a blockage in the stage 5 duct and the woma gun slipped out of hand allowing it to move around due to the trigger potentially being stuck. Causing a puncture and laceration wound to right and left thigh.

EE was working in the control room of the VLE Pilot Plant monitoring the plant. A bee got in the control room, landed on employees hand and stung EE. Employee finished shift, reported for shift the next day and after shift EE called EE's supervisor and told supervisor of the bee sting and that EE's hand and arm had some swelling so supervisor advised EE to seek medical attention.

Stepped on a small crate to get up to a higher pallet to count bags of coal for inventory. The small crate was empty and cracked when employee stepped on it. The crate flew up and hit ee and ee fell down and landed on the broken crate. Broke 4 ribs.

Employee was operating a Diesel Scoop about 30' from the top of the slope. The scoop died and rolled down the entire slope and hit a concrete wall at the bottom. The employee had a bump on head and was complaining of back pain. Employee was taken to hospital seen and released. This accident did not become a lost time until 11/28/17 when Doctor took employee off work.

The employee stepped unto a metal cover with left foot and felt a twinge in left knee. This incident was initially just a worker's comp claim since the employee continued to work without restrictions. On 2/16/18, the employee underwent surgery for a torn meniscus.

Employee claims to have strained left shoulder while pulling rock dust hose through a mandoor. EE completed shift and asked to complete an incident report as a precaution. Employee called off work on Monday, November 6, 2017 citing the ""injury"".

Employee was shoveling sand under a waste belt and caught employee's right pinky finger between the shovel handle and an angle iron (angled metal support) on the belt.

While loading under a shovel, the shovel bucket opened prematurely dropping material abruptly on the tail of the truck bed. The operator experienced back pain due to the impact. Treatment was limited to first aid and the employee continued to work without restrictions until 12-22-17, when the employee received light-duty restrictions from physician.

Injured got a foreign body in EE's eye and had to go to eye doctor to have eye cleaned out. At the doctor it was discovered EE had a laceration from the foreign object and received a prescription for EE's eye.

Employee stated that the man-trip hit a hole and jarred their back. Pain started going down the right leg immediately. This resulted in sprain/strain and lost work days.

Ee reports was driving haul truck down ramp and swerved to avoid a rock, striking the rib, causing ee to hit left rib/chest wall on the armrest of the truck. Seen at clinic, diagnosed with a chest wall contusion and put off work.

Employee was assisting with patches being hoisted to fix duct work. Employee was on the 5th step of a 12 foot ladder assisting. While employee was on the ladder the ladder twisted and the employee fell to the ground landing on left wrist. Employee was taken to the emergency room and was transferred to larger hospital for surgery. Employee suffered a broken radius and Ulna.

Initial treatment was diagnostic in nature only, returned to full duty. Employee received physical therapy for shoulder contusion on 12/14/2017. Injury became reportable on 12/14/2017.

EE was up on the plant grizzly welding structure together. After running out of welding rod, stood up & proceeded across beams. When ee reached the big beam, foot slipped causing ee to lose balance, fall into straddled sitting position. This caused right leg to go one side of the beam & left twisted. EE was loading the mobile conveyor, EE was chaining the front of the conveyor back by the tires on the lowboy trailer and as EE went to step off the back of the lowboy EE slipped and fell. 11/27/2017 Therapy was ordered and MSHA was filed.

The IE was moving an L shaped piece of cut stone when part of the stone broke loose and fell towards the floor striking ee on the foot just above the safety toe protective area. The piece of stone struck only one foot.

While working on removing skidloader bucket, bucket fell and struck IE right knee causing bruising. IE immediately released to restricted light duty. Employee was removing a 190 float machine. The machine was suspended from the float deck crane, employee was adjusting the machine with a come a long to facilitate removal from the cell. EE was hooking a chain from the loader to the dredge. EE told the Loader Operator when EE was done and to go ahead and backup. On 11/15/2017 an EE was separating sections of 4"" waterline with a crib block. The crib block slipped off of the joint of water line and EE struck finger between the crib block and a fire valve. The EE fractured the tip of finger.

A second set screw prevents the post from going all the way through the case erector. The operator let go of the post causing it to slide through the hole it rides in, and the above stop fell and cut the operators hand.

EE and another ee were unbolting a HDPE slurry pipe feeding de-watering screen SC-03A. The HDPE pipe has a slip flange on the end for ease of alignment. When the bolts were loosened the heavy HDPE pipe slipped and turned impacting ee in the chest knocking ee down and breaking collar bone.

Employee was using pry bar to dislodge decking plate. Decking plate broke free and ended up pinning the pry bar, which resulted in pinning employee's hand between pry bar and side of chute.

Unknown to IE, a piece of molten metal fell, hit IE's blue jeans and caught left lower pant leg on fire. IE's coworker noticed and smothered the fire. The result was an approximate 3-inch diameter second degree burn.

An employee using a metal hammer, connecting a pin into the links on a conveyor chain sustained a puncture wound that was caused by a piece of metal that had come from the pin as it was struck by the hammer, in EE's thigh. After, EE became aware of this, a few days later, EE was taken for medical evaluation where it was determined that the metal fragment should be removed.

Employees were reassembling guards on a belt conveyor. The injured was sliding a guard back into place when left hand was pinched between the guard and a handrail resulting in a contusion. The incident became reportable on 1/2/2018 due to physical therapy being assigned.

Employee slipped on screen plant and fell injuring their shoulder and back. Employee went to Dr. and was placed on light duty. Employee was trained in fall protection and had access to fall protection but did not wear it at the time of the accident.

EE was pulling a rolling dumpster backwards below the Clinker Cooler towards the #5 South Hopper when EE pinched EE's left hand between the rolling dumpster and a support beam causing a one-inch open wound on the side of EE's left hand below EE's pinky finger. EE received stitches to close the wound. EE was wearing EE's work gloves at the time.

While trying to dislodge the beam left index finger was caught between the beam and sideboard resulting in a contusion and laceration requiring 7 sutures. EE next scheduled shift was 11/27/17.

Employee was prepping to remove left front wheel on HT425. They attached the Ram-Holder to the A-Frame, and bent over to pick up extension. The Ram-Holder came off the A-frame and struck employee in the back of the head. Causing a 3/4"" soft tissue laceration. Employee was treated at the ED and received 3 staples.

The mine loader operator was returning to the high wall when right arm hit the bucket controllers causing the bucket to drop to the ground, and resulting in a sudden stop of the loader. Due to the sudden stop the employee was thrown into the windshield, causing injury to the right eye.

Injured was loading belt structure on the surface for a future belt move. Injured was in motion with a piece of structure and caught left thumb between the piece the injured was holding and another stationary piece of belt structure that was lying beside the one the injured was holding. Employee was assisting on the roof bolter. While the right side operator was putting in a rib mesh, injured tried to grab a bundle of plates that had slid under supply tray. At the same time the operator lowered the mast and pinched the employees hand.

EE was bolting top in #1 heading when a piece of draw rock fell and struck EE's left arm, EE had 10 stitches in left arm.

Employee was putting full plaster buckets through a man door at a stopping when ee left a pain in neck.

EE was changing out belt rollers. While EE and another EE had a top roller raised to install, it slipped out of EE's hand and landed on EE's right hand that was between the roller and the belt framing. This caused a laceration and a fracture to the tip of EE's ring finger on the right hand. 2 stitches were needed to close the wound and a finger splint for the tip of the finger.

Employee pushed stone slab onto loader forks and thought it was stable. After ee turned to look back at the saw, ee placed hand on the next slab and the previous slab (on the loader forks) came sliding off and crushed ee's pinky finger between the two slabs.

I had changed the cables on a crane from a single to a double. The crane operator boomed up as my hand was on one of the cables causing might pinky on my right hand to be pinched between the pulley and cable.

A Mine employee was positioning a relay bar being installed on the longwall set-up face and supported by a come along (chain style). While positioning the relay bar, the come along chain broke and struck the employee in the face. The employee sustained a laceration wound on the chin that required stitches and a tooth was also damaged. The employee has been released to work.

While using a track jack to re-rail, a mobile jack slid sideways causing employees finger to get pinched between the track jack and mobile. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency attitude or machinery.

EE was replacing a spout plug in the shipping silos spout. When EE looked up to align the plug with the bolt holes dust fell into EE's eyes. EE was wearing safety glasses and hard hat. EE had lowered the spout and knocked any loose material out before beginning work.

Individual was operating torch to cut sections of plate. While checking how much remained to be cut, the piece came loose of the structure, pinching the upper ring finger against plate below. This caused a laceration through the welding glove as EE pulled the hand free. The laceration required three stitches. X-rays were forwarded to radiology and resulted in negative findings.

Employee was cutting open a transmission filter to check for metal cuttings. EE's hand made contact with the sharp edge of the filter and caused a laceration to left thumb.

The mine employee was removing metal shingles from the No. 1 kiln and cut left hand on one of the shingles. It required 6 stitches.

A fully trained miner injured right knee when walking the 5 North #2 Belt. When walking ee slipped and struck knee on the bottom. Employee received Medical Treatment for the injury. The accident was not the result of any compliance issues nor were any protective guards involved.

The crew was installing a bearing on the stacker conveyor. A maintenance worker used a 6 foot pry bar to lift up the shaft to access the bearing. The pry slipped off the shaft and the worker's right shin struck the toe board below. Note: The injured employee received initial treatment for the injury on 11/16/2017.

Employee was working on the F-27 Longwall face when a rock fell from between the shields and grazed the left side of face causing a laceration to left cheek that required 5 sutures to repair.

Employee had just finished servicing the shovel in the pit and was waiting in the lube truck for a loader to finish maneuvering so they could safely exit the pit. While watching the loader and waiting truck was hit from behind by a backing dozer. Employee suffered back pain from the jarring impact of the dozer. Employee did not receive prescription medication until 11/10/2017.

EE was operating a hydraulic rock breaking hammer attached to an excavator when EE was struck in the face with a flying rock. The machine has same guarding and it is unknown when the screen was removed or why. The EE didn't notify anyone in authority and left jobsite early, and later on is when boss talked to EE on phone that night. Never did tell boss of injury.

A new production control supervisor (PCS) was assigned the task of changing valve positions in the buffer silos. The PCS used the incorrect tool for the task (too large). When tapping the actuator ee struck/pinched the tip of right ring finger between the tool and a pipe flange. The injury is a laceration and fracture of the finger tip.

While assisting in carrying a wire cable, both employees slipped and fell due to mud conditions. Employee's wrist was cut by the cable by employee falling. Went to Princeton Community Hospital for attention. Treated and released after application of adhesive in lieu of stitches.

The IE stated to the operator IE was struck by a rock that fell from between the shields and struck hand. Stated rock was 6""x 8""x 2"" and IE's hand was on the cable tray.

EE pulled scoop up to load & turned scoop off & dropped bucket on ground. When exiting scoop began to roll back causing ee to twist and fall. EE complained of pain in left knee. EE has not returned to work since the day of the DOI, 11/6/17.

EE was backing up the water truck towards a berm. EE went too far and started to drive up the berm, shifting the weight of the water tank, causing the truck to roll on its side

Contractor came on site to update Ethernet switch. Contractor was hit in head by an Ethernet switch requiring medical treatment. Contractor received 5 sutures in back of head and returned to finish job. This is not our employee and not required to have an MSHA ID number.

EE was tramming a scoop thru a set of double doors @ 81 Brk Intake toward the belt line to rock dust. After pulling thru 1st set of doors, EE arrived at the 2nd set of doors and started to open them when scoop started rolling back toward 1st set of doors which the scoop hit causing door to go across EE's body in the deck of the scoop scraping back and catching arm against scoop.

Employee was grinding a shim plate on top of the kiln (9' grinder). The grinding wheel flew off of the grinder, struck the Tyre, deflected back and struck employee in the right thigh. First aid was administered. Employee was taken to urgent care where they received 5 stitches to right thigh.

The employee was in the 3A longwall in the process of changing bits on the tailgate drum of the shearer. While the employee and a co-worker were spinning the drum down, the injured employee pinched right ring finger between a newly installed bit and the LW face. The result of this incident was 14 sutures to the right ring finger.

Mechanic was in the process of changing the teeth on an excavator bucket. The teeth were difficult to remove and required the use of a sledge hammer to work them loose. While hammering the one of the teeth loose, a small shard of metal broke away on impact and imbedded into employee's left leg below the knee.

EE was trying to loosen nut on Eagle Sand Screw. Nut broke loose. In process the wrench pinched finger against frame.

EE was removing a bolt using a 1/2"" electric impact wrench. EE was holding the bolt head with left thumb and index finger. The bolt head spun and lacerated ee's left thumb which required three sutures.

EE was working on switching a slurry pump. When the pump started the discharge line immediately blew apart at the 10 inch roller grip clamp spraying three of the employees standing off to the side of the pump. The EE was trying to get out of the spray and made contact with a pipe manifold located three to four feet away. This resulted in a laceration above right eye.

Injured was swinging a sledge hammer and strained a muscle in lower back, on 11/9/2017 injured was seen by doctor and prescribed a muscles relaxer for back.

The employee was assessing the damage to the fan assembly of a Volvo L220E loader. The fan assembly suddenly fell and the employee put hand out to prevent the fan assembly from striking the employee. A sharp edge on the fan assembly caused a laceration to the back of right hand that required sutures.

Mechanic was removing a guard between the transfer case and engine on a kress coal truck. They removed 1/2 half of the guard, while handing it to the other mechanic they brushed against the other half embedding a piece of metal in the left elbow 1/2 inch with of human hair, one stich required for removal.

A mechanic received minor burns to the hands from a fire which occurred in the control room of the Rossi Crusher. A blister on the left wrist was removed, treated, and dressed by a physician. The employee was released to full duty.

Employee was helping install a cutting edge on the motor grader. EE right hand index finger and ring finger was pinched between the cutting edge and the mold board. Stitches were required.
