ROOF FALL AT MOUTH OF 1ST LEFT INTAKE, CONTINUATION OF FALL IN INTERSECTION AND CROSSCUT KNOCKED OUT PART OF STOPPING, OVERCAST-CRIBBED AND FIXED STOPPING. 40'LONG X 20'WIDE X 6'HIGH.
CONTINUOUS MINER WAS MINING IN #2 ENTRY ON #2 UNIT. HE HAD MINED THE RIGHT SIDE AND HAD SET OVER AND WAS MINING THE LEFT SIDE AND WAS IN ABOUT 17-18 FT. OPERATOR SAW THE GLOW AROUND THE DRUM AND STOPPED CUTTING & THE GLOW DISAPPEARED. MINING CONDITIONS CONSISTED OF MINING ROCK & SULPHER BOLLS ABOVE THE COAL SEAM.
A ROOF FALL OCCURRED IN THE MAIN WEST IN THE #6 ENTRY BETWEEN CROSSCUT 34-35. THE FALL MEASURED 20 FEET LONG, 18 FEET WIDE AND 6 FEET HIGH.
ON FRIDAY, JULY 30, 2004, A ROOF FALL WAS DISCOVERED AT THE BOTTOM OF THE SLOPE AT A PREEXISTING OVERCAST ON THE MAIN LINE. THE FALL MEASURED 4' WIDE X 8' LONG X 15' HIGH. THE FALL HAS BEEN INSPECTED BY MSHA INSPECTOR.
A FALL WAS DISCOVERED IN THE #2 ENTRY OF 2 RIGHT OFF 2 MAINS WEST ABOUT 20' INBY SURVEY STATION 920. THE FALL MEASURED 60' LONG X 6' HIGH X 15' WIDE. THERE WERE NO INJURIES AND NO EQUIPMENT WAS INVOLVED. VENTILATION WAS NOT AFFECTED. THE FALL IS APPROX. 15,000 FEET OUTBY THE ACTIVE SECTION.
An unintentional roof fall occurred in our 4 south belt entry @ 4 wall. The fall measured approx 16' by 12' by 30' l. No injury or equipment damaged.
Roof fall in #3 entry at spad No. 451. Fall was 35' x 20 x 3' to 5'. Fall is 5500' outby active work section. No airways blocked or travelway. No plans to rehabilitate.
A roof fall occurred at the above mentioned mine. exact time is unknown. location was spad 2386-face inactive. no personal injuries. the size of the fall was approx. 25' x 18' x 7"" supported by torque tension bolts, strata being wet shale, and also has a 2 ft rider scam, this area will see rehab.
An unplanned roof fall occurred at the justice mine tailgate section No. 4 entry Bk 116 to 117. The fall measured 20' by 18' by 7'. The date and time of this fall is unknown. It was discovered 7/29/04.
An unintentional roof fall occurred at the millenium portal at the 5N longwall headgate entry #3 (belt entry) between crosscut #46 and #47 at 3,538N. The fall was approx 35' long approx 17' wide and approx 12-15' thick. Did not affect ventilation, will be cleaned up.
A roof fall occurred at the above noted mine at locationg-1 break inby spad 2375. Face is inactive. no personal injuries occurred. the fall measured 18'x 18' x 7' in size with torque tension bolts as support in shale. there is rider seam of approx. 24"" x this area will not see rehab.
A roof fall occured in an intersection in the Mill Fork Left section. The fall measured approximately 20' x 20' x 6' in height. This roof in this area is laminated mustone and cap rock. Roof fall was in rock rooms and was not part of the working faces on this section.
A FALL OF ROOF OCCURRED IN THE S.E. MAINS LOM C BELT ENTRY AT X-CUT 172 ON THE 7TH 54 BELTLINE. THE FALL WAS 25' X 18' X 6. THE FALL WAS LOADED OUT AND PINNED. NO VENTILATION OR PERSONELL WERE AFFECTED.
THE GARDEN B-SHAFT AUTOMATIC ELEVATOR WAS INOPERABLE DUE TO AN ELECTRICAL MALFUNCTION IN CONTROL CIRCUIT.
After completing the hoist pre-operation inspection, the hoistman found the hoist motor would not start. Electricians troubleshooting the situation found the main feed breaker linkage required adjustment before the breaker could function properly. MSHA notified immediately and hoist was operational.
A ROOF FALL OCCURRED IN THE FACE HEADING OF THE 5 LEFT CHUTES. IT OCCURRED BETWEEN THE FIRST AND SECOND X-CUT. THE FALL WAS APPROX. 70 FEET LONG, 15 FEET WIDE, AND 6 TO 10 FEET ABOVE THE BOLTS. THE AREA WAS DANGERED OFF AND IS BEING POSTED OFF.

A ROOF FALL WAS FOUND IN A 4-WAY INTERSECTION OF A SET OF WORKED-OUT RETURN ROOMS BY EXAMINER DOING THE WEEKLY AIR COURSE INSPECTION. THE LOCATION WAS ROOM 3 SURVEY STATION 2+40, 5RMS. FALL WAS 20' X 20' X 10' HIGH. AREA WAS MINED 1/15/04 AND HAD BEEN SUPPORTED WITH 6' AND 8' FULL-RESIN BOLTS. TRAVEL ROUTE WAS RELOCATED. NO PROPERTY DAMAGE OR INJURY RESULTED.
On initial start up 8' lift was taken and channels and bolts were installed to within 3' of face rain was contributing factor to fall erosion on high wall this fall was in belt entry of main heading.
A ROOF FALL OCCURRED IN THE 2ND MAIN WEST IN #3 ENTRY AT #42 XCUT. THE FALL MEASURED 50' LONG, 18' WIDE & 12' HIGH.
An unintentional rock fall was discovered in #5 entry at #11 break. Size of fall estimated: 4 1/2'H x 20'W x 20'L. Area had been supported with 4' fully grouted resin bolts with additional support consisting of 10' cable bolts and cribbs. No injuries, no equipment caught.
Roof Fall above the bolt anchorage #8 Entry of 2 Rt 2 xcuts inby the intersection of 1 Rt #8 Entry the area is in the return and did not impead ventilation.
A roof fall occurred in entry #1 of the main north east at survey station 4+90 to 5+60. The fall masured 120' long x 18.5' wide x 9' thick. The area had been bolted with 9' double locks and 6' fully grouted bolts. All approaches have been timbered off.
A ROOF FALL WAS FOUND IN ENTRY #2 SURVEY STATION 39+90 5RMS UNIT. FALL WAS 50' LONG X 20' WIDE X 10' HIGH. THE FALL OCCURRED IN 4-WAY INTERSECTION THAT HAD BEEN MINED MAY 15, 2004. AREA WAS SUPPORTED WITH 6 AND 8 FOOT FULL-RESIN BOLTS. NO INJURY OR PROPERTY DAMAGE RESULTED.
A FRICTIONAL IGNITION OCCURRED AT THE FACE OF #11 ENTRY INBY #120 WALL, 102' FROM LAST OPEN CROSSCUT, ON THE 3 NORTH RIGHT SECTION. THE IGNITION LASTED APPROX. 5 SECONDS AND WAS EXTINGUISHED WITH A WASH DOWN HOSE AND WATER SPRAYS ON THE MINER. THERE WERE NO INJURIES OR DAMAGE TO EQUIPMENT.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE MILLENNIUM PORTAL AT THE 5N LONGWALL HEADGATE ENTRY #3, 4818N AT CROSSCUT #55 (BELT ENTRY). THE FALL WAS APPROX. 46'LONG X 28'WIDE X 20-25'THICK. THE FALL WILL BE CLEANED UP.
THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES WITH PEOPLE ON THE ELEVATOR. THERE WAS NO DAMAGE TO THE ELEVATOR. THE ELEVATOR OUTAGE WAS CAUSED BY THE LIMIT SWITCH FOR THE GOVERNOR ROPE - HAD TRIPPED BECAUSE THE NEW ROPE HAD STRETCHED.
A ROOF FALL WAS FOUND AT THE 1ST MAIN WEST INTAKE ENTRY #2 AT 72 XCUT. THE FALL IS APPROX 40' L X 18' W X 10' H. THE FALL WAS SUPPORTED WITH 10' POINT ANCHOR ROOF BOLTS. THE FALL WILL NOT BE CLEARED UP.
A ROOF FALL OCCURRED AT XCUT 12, ENTRY #4 SECONDARY ESCAPEWAY, MAIN NORTH OFF OF THE MAIN WEST. FALL IS 16'L X 18'W X 6'H. SUPPORTED WITH 6' POINT ANCHOR BOLTS. FALL WILL BE CLEANED UP AND RESUPPORTED. SECONDARY ESCAPEWAY HAS BEEN REROUTED.
A roof fall in the 2 Main North in the intersection of cross-cut #268 in #7 entry measuring 40 ft. x 40 ft. x 6 ft. The area was mined in December 2004 and supported with 6 ft. tension rebar bolts. This fall was caused by an undetected slip.
The slope hoist at our Wheatland portal malfunctioned. It would only travel up and would not lower into the mine. It was found that a safety switch located near the hoist drum had been tripped which caused the malfunction. The switch was reset and the hoist was returned to operation.

ROCK FALL IN THE # 5 ENTRY AT THE 54TH CROSS CUT APPROX 6' LONG 5'HIGH CRIBBED AND TIMBERED ALL SIDES OF FALL AND DANGERED OFF.

Sub-contractor was operating a company owned 50 ton haul truck. At the start of the shift he moved the haul truck with the bed in the raised position. The raised bed came into contact with overhead 15,500 volt high voltage power lines. (per call - driver told to remain in truck until power company arrived at 8:10 a.m to determine that power was off).
A ROOF FALL OCCURRED OUTBY ON #2 UNIT. THE FALL WAS IN AN INTAKE ENTRY. THE AREA WAS SUPPORTED WITH 10' POINT ANCHOR ROOF BOLTS. THE DIMENSIONS OF THE ROOF FALL ARE APPROXIMATELY 50' LONG X 18' WIDE X 10' HIGH. THE ROOF FALL WILL NOT BE CLEANED UP.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE MILLENNIUM PORTAL AT THE 4N LONGWALL AT ENTRY #2 (6056N, 925W). THE FALL WAS APPROX. 18'LONG X 18'WIDE & APPROX. 15'THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF PEOPLE.
A thunderstorm came through the area knocking out power to the Shoals Plant. The mine hoist was disabled. The mine evacuated due to only one means of entry/exit.
#2 return entry at spad #1921 approx. 2000 outby the working section approx. 35' long x 19' wide x 5' thick. slate appears to have separated from the sandstone in the roof 5/8"" x 48"" fully grouted rebat-bolts with 8"" x 8"" plates
End dump trailer tipped while dumping into crusher bin. Inspection showed 8' tilt in surface area and 25+ mph wind contributed to tipping of trailer. No injuries. Had area leveled.
In # 3 entry in by spad #3804 lowering the miner up. No one was injured by the fall.
AN IGNITION OCCURRED AT THE FACE OF #11 ENTRY INBY 120 WALL ON THE 3-NORTH RIGHT SECTION. THE IGNITION WAS EXTINGUISHED BY WATER HOSES FROM BOTH SIDES OF THE MINER. THERE WERE NO INJURIES OR DAMAGE TO EQUIPMENT.
EMPLOYEE WAS RETURNING TO THE HYDRAULIC SHOVEL FROM DUMP AREA WHEN HE SAW SMOKE AND FLAMES COMING OUT FROM THE ENGINE DECK. HE GOT OUT OF THE CAB AND DECIDED TO GET DOWN OFF THE TRUCK. THE TRUCK HAD EXTENSIVE DAMAGE TO THE ENGINE COMPARTMENT, CAB, TIRES (FRONT), HYDRAULIC SYSTEM AND ELECTRICAL. DRIVER WAS NOT INJURED. TIRE BURNED ABOUT 1/2 HOUR.
#5 hoist down from 1:45 to 4:05 on this date. This was an electrical problem due to lightening. All other hoists operable.
The cedar creek service elevator was out of service from 1:30 am until 12:15 pm. A power supply board had to be replaced.

A roof fall was found at 11:30 am on 7-20-04 by examiner making weekly air course. The fall was in a 4 way intersection of entry 5, x-cut 38 main west. The fall was 20' long by 20' wide by 10' high. Area had been mined July 17, 2001 and was supported with 8' full resin bolts. No injury resulted.

AN UNINTENTIONAL ROOF FALL OCCURRED AT SPAD #44+40 ON THE 3RD SOUTHEAST SUBMAIN, #3 UNIT, #3 ENTRY. THE FALL MEASURED 20' X 40' X 10'. THE FALL HAS BEEN TIMBERED OFF AND DANGERED OUT. THE FALL WILL BE CLEANED UP.
DRIVER DUMPED HIS LOAD & PULLED OUT OF STOCKPILE AREA W/HIS TRUCK BODY IN THE UP OR DUMP POSITION. HE TRAVELED APPROX. 170'BEFORE CROSSING ONTO MINE PROPERTY AGAIN & TRAVELED APPROX. 30'BEFORE CONTACTING 600 VOLT OVERHEAD POWER LINES. THE DRIVER REMAINED IN THE TRUCK FOR APPROX. 45 MINUTES UNTIL CONDITIONS WERE DEEMED SAFE FOR HIM TO EXIT. HE WAS NOT INJURED.
An unintentional roof fall occurred at survey station 20818 and extending into the 5 rt l.w. face cut out entry. Fall is 20 ft wide, 60 ft long and approx. 20 ft high. This fall will be cleaned up, using 5 rt l.w. equipment. There were no injuries.

ROCKFALL AT SPAD # 4290 ON 2 LEFT MAINS IN # 2 NEUTRAL ENTRY 8 BREAKS BELOW # 3 DRIVE. FALL STARTED IN NEUTRAL & RODE OVER TO BELTLINE & CAUGHT ABOUT 1/2 OF THE BELT. FALL WAS APPX 18-20'WIDE, ABOUT 130' LONG & APPX 6-12'HIGH. BOLTED WITH 4' RESIN BOLTS & APPEARS TO BE AN UNDETECTED FAULT AREA WITH SOME PRESSURE FROM BLOCKS OF COAL LEFT IN THE HIGHSPLINT SEAM ABOVE.
A roof fall occurred in entry #1 of the first S.E. mains at survey station 17 and 50. The fall measured 60' long by 18.5' wide by 9' thick. This area had been supported with 9' double locks and 6' fully grouted bolts. All approaches will be timbered off.

A roof fall occurred in #5 entry of the 2nd main south at survey station 4+90 to 5+60. The fall measured 70' long x 18' wide x 6' thick. This area had been bolted with 5' fully grouted resin bolts. All approaches have been timbered off.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #2 ENTRY, AT THE #8 X-CUT LOCATED IN THE PANEL 8 EAST RETURN AIR COURSE. THE ROOF FALL WAS 40'LONG X 18'WIDE X 8'HIGH. SUPPLEMENTAL SUPPORTS HAVE BEEN INSTALLED AROUND THE ROOF FALL & THE ROOF FALL WILL NOT BE CLEANED UP. INITIALLY BOLTED WITH 4'FULLY GROUTED ROOF BOLTS. THE ROOF MATERIAL CONSISTED OF LAMINATED BLACK SHALE & A SAND STONE.
FOUND ROCK FALL IN #2 UNIT INTAKE IN #8 ENTRY FROM 10 FT OUTBY 19+90 TO 21+20 PLUS. DO NOT KNOW WHEN IT FELL. NO WATER COMING IN BUT PULLED 6' RESIN BOLTS. FOUND WHILE DOING ROCK DUST SURVEY. 130 X 19 X 5 1/2.
A roof fall in the 1 South in the intersection of cross-cut #47 and extending to the edge of the intersection of cross-cut #49 in #3 entry measuring 140 ft. x 19 ft. x 6 ft. The area was mined in April 2004 and supported with 6 ft. tension rebar bolts. This fall was caused by a hidden slip in the mine roof.
An unintentional roof fall was discovered on grassy creek mains #1 entry, between survey station 18751 and survey station 18801. fall size is approx. 20 ft wide x 40 ft long x 7 ft thick. there were no injuries associated with this incident. This fall will not be cleaned up.
The pre-shift examiner found a roof fall along the pillar extending from the main entry to 1st right (later falling to 2nd right). The fall extended above the 42 inch mechanical bolts. The area was timbered and dangered off.
SLOPE CAR WAS INOPERATIVE DUE TO A FAULTY FAST/OVERSPEED SWITCH. SWITCH WAS REPLACED AND SLOPE CAR WAS PUT BACK INTO OPERTION AT 7:15 AM.
EE WAS PERFORMING WEEKLY EXAMINATION OF AIRWAYS WHE FALL WAS DISCOVERED. FALL OCCURRED BETWEEN 7-12 AND 7-19-04. FALL IS IN #8 ENTRY 80' INBY SPAD #150 IN INTAKE. AIRWAY NOR EXCAPEWAY IS BLOCKED. WE HAVE 2 INTAKES IN THIS AREA. AREA IS TIMBERED OFF AND WE DO NOT PLAN TO RE-HABILITATE AREA OF FALL.
UNINTENTIONAL ROOF FALL IN THE 2EP UNIT #4 ENTRY AT XCUT #76. 10'LONG, 18'WIDE 5' HIGH. FALL WAS NOT INBY TAIL BUT ADJACENT TO TRAVELWAY. 050-0 MMU WAS BOLTED WITH 5' CONVENTIONAL PINS.
A roof fall occurred on our west 3 section in the #4 entry at spad 1 and 20. It measures 20 ft long and 20 ft wide and approx 9 ft high. It is located approx 150-175 ft out by the face.
THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAT 30 MINUTES WITH PEOPLE ON THE ELEVATOR. THERE WAS NO DAMAGE TO THE ELEVATOR. THE ELEVATOR OUTAGE WAS CAUSED BY TWO BLOWN FUSES (ONE TO THE UPS AND ONE TO THE DRIVE).
Rock fall occurred 56 breaks inby the Big #4 Head-drive, #5 entry (track entry) of the South Mains. Rock fall measured eighteen feet by seventy feet by one and a half feet to three feet thick. Ventilation was not impaired.
UNINTENTIONAL ROOF FALL OCCURRED OUTBY SPAD #6034 BETWEEN THE 18TH & 19TH X-CUT IN THE #4 ENTRY. FALL AREA WAS 20'WIDE, 30'LONG, & 3-4'THICK.
ON JULY 14, 2004 AN UNPLANNED ROOF FALL WAS DISCOVERED IN GRASSY CREEK NO. 1 MINE. THE FALL OCCURED IN #15 BELT AND SPAD NO. 5379, 1 BREAK INBY. THE SIZE OF THE FALL MEASURED 45 LENGTH 20 WIDTH 5 FT. IN HEIGHT. THE AREA HAD BEEN BOLTED WITH 4 FT RESIN BOLTS.
Continuation of old fall - App. 7 ft high, 25 ft long, 14 ft wide - No injuries, no equip, no blockage of air or escapeway. Will not be cleaned up - Breakered off & dangered, plotted on map.
A friction caused methane ignition on the face of the longwall panel 49 near shield 144/ A blue and orange flame was observed as the shearer mined the fourth pass of the shift from the tail to the head. The flame lasted two to three seconds and was extinguished by the sprays on the shearer drum and spray bar.
In the upper seam, a 50' long by 20' wide by 5' high fall occured in No. 4 entry 3,500' from the face in the lower seam. This area of mine is used for a belt conveyor in the upper seam, in a neutral entry away from the belt line and has been breakered off.

The area of cutting and welding had been wet down, dusted with up to 4 inches of rock dust and fire retardant foam applied 15-18 inches thick. Project had been in progress about 3 hours (replacing headgate spray boom). A spark ignited methane on the floor on the inby side of the lowered cowl. It was immediately extinguished. This occurred about shield 10 of the face.
ON JULY 16, 2004 AN UNPLANNED ROOF FALL WAS DISCOVERED IN GRASSY CREEK NO. 1 MINE. THE FALL OCCURED #17 BELT, SPAD NO. 6017, 1 BREAK OUTBY. THE SIZE OF THE FALL MEASURED 20 X20 LENGTH, 20 FT WIDTH AND 84"" HEIGHT. THE AREA HAD BEEN BOLTED WITH 4 FT RESIN BOLTS.
Roof fall measuing 120' L X 19' W X 5.5' T was found in neautral entry during an exam. The area was developed Jan. 04 and was 2,200' from the working face. The area was bolted with 5' tension roof bolt, the fall occurred due to coal streaks intermixed with slate. Ventilation the passage of men was not affected, no person were injured. Fall area was barricaded to prevent travel.

THE AREA WAS DANGERED OFF WITH CRIBBS AND TIMBERS.

THE #1 HYDRAULIC PUMP FOR THE BRAKE GOT WEAK AND COULD NOT PUMP THE REQUIRED 2300 PSI TO SHUT OFF THE HYDRAULIC SYSTEM. THE PUMP CONTINUED TO RUN, CAUSING THE SYSTEM OT OVERHEAT. THIS CAUSED THE TANK PLUG TO MELT AND DAMAGED SENSORS, CAUSING THE SYSTEM TO FAULT OUT. AS A RESULT OF THE FAILURE, THE HOIST WAS INOPERATIVE FOR MORE THAN 30 MINUTES.

Hoist Brake accumulator failed on the production shaft at 11:25 AM. The mine was evacuated at 11:55 AM. Repairs were completed at 1:00 PM.
WHILE CUTTING FROM HEAD TO TAIL THE SHEARER HIT A SANDSTONE INTRUSION. WHILE CUTTING THE INTRUSION AN OPERATOR STATED HE SAW A YELLOW FLAME THAT LASTED 5-10 SECONDS. THE FLAME SELF-EXTINGUISHED.

Suspect that lightning struck during a severe thunder storm causing ignition of coal dust inside the dust collection baghouse atop silo 3. A computer time stamp of 6:53 p.m. indicated miscellaneous and unconnected electrical disruptions. Baghouse doors found open and bags gone at 12 noon on 07/14/04. Investigation began into how, what and when.

An excavator operator was cleaning out and sump when the tooth of the bucket ruptured the pipeline. the excavator was out off and the gas company was immediately watified. this accident occurred off of mine and permitted property.
A power outage occurred at approx 8:50 pm. which resulted in a single phase condition. this condition caused failure of the breake pump motor for the mini cage at the production shift. 3 men were on the mini cage for 4 1/2 hours due to this condition.

THE FLAME WAS REPORTED AS LASTING TWO TO THREE SECONDS AND WAS EXTINGUISHED BY THE DRUM SPRAYS AND SPRAY BAR.

THE ROOF FALL WILL BE CLEANED UP AND THE AREA RE-SUPPORTED. INITIALLY THE AREA WAS BOLTED WITH 4' AND 6' FULLY-GROUTED ROOF BOLTS.
A roof fall occurred in the 1 Right Section MMU-001. The fall occurred in the #1 Entry of 1 Right Section. The fall began just inby Station #1902 and extended inby approximately 150'. The area was bolted with 4' point anchor resin bolts. Fall occurred above the anchorage of the bolts. No men or equipment were involved. Ventilation was not interrupted.
Rock fall occurred in number 4 entry of the South Mains 73 breaks inby the Big Number 4 head drive, fall was on the return side. The fall measured 18 feet wide, 25-30 feet long and approximately 4 feet thick. (No ventilation was obstructed.)

A ROOF FALL WAS FOUND MEASURING 6 - 7' THICK IN THE NO.5 ENTRY 50' OUTBY SS#353. EST LENGTH FALL 50' X 20' X 6-7' APPROX 6000'SURFACE 500' OUTBY SECTION.
An alarm sounded at 10:00 PM on the cage hoist. The hoist safety system engaged and the hoist did not move after the alarm sounded. A broken, brake pull-rod on one of the four brakes was discovered at approximately 12:15 AM. The replacement repair was completed at 2:35 AM on July 6.
There occurred a roof fall at this mine located at #3 entry inby spad #362 about 2160ft from the working face. It fell on highline cable in the intake. The size of the fall was 20ft L x 20ft W x 6ft H. Conventional 4ft bolts were used and strata was laminate. Ventilation was not affected and there will be no rehab.

A ROOF FALL OCCURED IN THE 1ST S.E.MAINS FROM ENTRY 0 TO ENTRY 1@ SURVEY STATION 27+50. THE FALL MEASURED 90'LONG X 18.5'WIDE X 9'THICK. THIS AREA HAD BEEN BOLTED WITH 9'DOUBLELOCK & 6'FULLY GROUTED BOLTS. ALL APPROACHES TO THIS AREA HAVE BEEN TIMBERED.
The discharge chute below crusher became plugged. the skip was dumped at this time. a high-hopper level limit switch was tripped that disables the hoist. the mine was evacuated due to only one means of entry/exit.

AN UNINTENTIONAL ROOF FALL OCCURRED AT E-MAINS IN #5 HEADING, BETWEEN SPADS 10368 - 10370. THE FALL MEASURED 18' WIDE X 35' LONG X 8' HIGH.
#15 and #16 heading shot into an old mine that was not shown on the mine mop, when the place was drilled it did not drill into the mine when the place was blasted it shot thru into the old mine. the old mine was not shown on any maps. no one was aware this area had been mined. no injury to persons or equipment
A rock fall occurred in the #8 entry 1 break outby spad # 1844. The fall measured 50'L X 20'W X 5'H.
A ROOF FALL OCCURRED IN THE ""A"" MAINS 1 RIGHT PANEL AT THE #9 CROSSCUT BETWEEN THE #2 AND #3 ENTRIES, ADJACENT TO SPAD #21897. THE FALL WAS APPROXIMATELY 5' HIGH X 19' WIDE X 50' LONG. THE FALL AREA HAD BEEN BOLTED WITH 5' BOLTS.
The S1 brake caliper had a bad seal in the piston of the lower cylinder (actuator). The brake shoe was dragging and chattering on the disc. The midnight crew was held out of the mine while the brake cylinder was replaced and tested. Mine was vacant at the time, MSHA recording notification 1:15am. Hoist functional at 2:00am.
There was an ignition on the 3-north right section at the face between #8 and #5 entry at #117 crosscut. the ignition lasted 2 seconds and self extinguished by the water sprays on the miner. there were no injuries and no damage to equipment
While cleaning material buildup from underneath field conveyor using a Bobcat loader, the employee climbed out of the vehicle to inspect a pin which had worked loose on the boom arm. After climbing back into the Bobcat, the employee bypassed the safety devices and operated the equipment standing up and became pinned between the boom crossmember and the roof of the cab.
EMPLOYEE WORKING NEAR EDGE OF HIGHWALL FELL 90 FEET AFTER SLIPPING OR STEPPING IN WRONG DIRECTION.
WHILE WORKING ON AN AIR LUBRICATION PUMP ON A WORK BENCH IN THE REFUSE SHOP, EE'S LEFT THUMB WAS SEVERED BETWEEN THE BOTTOM OF THE NAIL & THE 1ST JOINT. WHEN AIR WAS APPLIED TO TEST THE PUMP, THE PISTON STROKED, CUTTING THE END OF HIS THUMB WHICH WAS INADVERTENTLY IN THE CHAMBER.
Ee was attempting to clean out the rotary air lock feeder by using his index finger. Resulted in amputation of end of right index finger. Ee will be out approx six weeks.

THE EE WAS OPERATING THE MINER WITH A REMOTE CONTROL WHEN A ROCK APPROX. 10""X 10""X 1"" THICK CAME OUT OF THE ROOF & CAUGHT HIS RT. LITTLE FINGER BETWEEN THE REMOTE CONTROL & THE ROCK. THE END OF THE FINGER WAS AMPUTATED.
EMPLOYEE WAS USING A 12 LB SLEDGE HAMMER WHILE WORKING ON AN UNDERCARRIAGE. HE EXPERIENCED A SHARP PAIN IN HIS LOWER ABDOMEN.
Finish mill #3 feed pipe was choked. using pinch bar to get choke out bar got caught by mill and spun bar between bar and feed pipe which cut left hand finger tip off---called and amputated first joint of ring finger.
He was pulling on a log chain when he got a hernia.
Employee was using pry bar to knock down buildup inside vessel while standing outside inspection door. Buildup released suddenly causing stain in groin area. Small jackhammer was available to use, but employee chose not to. Told to use it from now on.

Changing a blow line from 1 pipe to another. There was no air pressure on the line when the accident occurred. The line slipped off the pipe, whipped around and hit EE in the face. EE has a deviated septum; will have open nasal reduction surgery 08/10/04.
EE WAS STACKING STEEL GIRTS FOR RIGGING TO FLY THEM INTO LOCATION W/ CRANE. THE I-BEAM FELL OVER ONTO THE OTHER GIRTS & STARTED A DOMINO EFFECT WITH THE GIRTS CAUSING ONE OF THE GIRTS TO FALL AND CUT THE END OF HIS FINGER OFF. HIS FINGER TIP WAS PROPERLY STICHED BACK ON BY A PHYSICIAN. HE WILL BE RETURNING TO WORK AUGUST 4, 2004.
Ee sprain back while lifting gears and other scrap metals out of a container.
EE HAD JUST SET ATRS ON ROOF BOLTER AND SAW IMMEDIATE ROOF CRACK. EE STARTED RAISING AND LOWERING ATRS IN ATTEMPT TO BREAK THE ROCK. AFTER SEVERAL ATTEMPTS THE ROCK BROKE FREE AND RODE BACK TO THE OPERATOR CONTROLS, STRIKING EE IN HIS LEFT SIDE LEG AND HIP AREA. ROCK SIZE WAS 30"" LONG X 20"" WIDE X 4"" THICK.

Employee claims to have injured his back while straightening a roof bolt for insertion. The bolt rotated with him while he was applying pressure- thus injuring his back. MINE MANAGEMENT BECAME AWARE OF THIS ACCIDENT ON 8/31/04.
EE WAS HAULING COAL WITH A SHUTTLE CAR AND A PIECE OF DRAW ROCK FELL OUT IN BETWEEN THE ROOF BOLTS, HITTING HIM IN THE LEFT SHOULDER AND SIDE.
Employee was laying out surface delays when he stepped over a surface trunkline with his right foot and buckled his knee. He lost his balance momentarily but didn't fall. His right knee twisted when the soft drill cutting gave way while transferring his body weight from his left foot to his right foot.
Employee was attempting to climb down off a belt line when his right foot became caught between a 2"" aluminum pipe and a timber. he fell to the ground landing on his back with his foot still caught by the timber and pipe. employee said he felt pain in his thigh area when he fell.

EMPLOYEE BENT OVER TO LIFT A TIMBER ONTO A CAP PIECE IN PREPARATION TO CUT IT WHEN HE FELT PAIN IN HIS BACK. BACK STRAIN, MEDICAL TREATMENT, PHYSICAL THERAPY.
Employee was laying on a 2"" highwall landing next to a dryer (cyclone) cleaning it. As he rolled over he lost his balance and fell off the landing scraping the side of his head on the landing resulting in a laceration.
Employee was injured while riding a mantrip under tow. Mantrip was jerked when scoop started pulling it, thus re-injuring employee. This is a re-occurrence of a 12/20/03 accident.
EMPLOYEE WAS ROOF BOLTING WITH A COBRA DRILL AT #72 SHEILD F-9 LONGWALL FACE WHEN A ROCK THAT MEASURED APPROX. 1'THICK X 2 1/2'WIDE X 15'LONG FELL & DISLODGED THE ROOF JACK THAT WAS SET. AS THE EE WAS MOVING OUT OF THE WAY. THE ROOF JACK STRUCK HIM ACROSS THE LOWER BACK CAUSING HIM TO FALL HITTING HIS NECK AGAINST THE RAIL OF PANELING. EE RECEIVED 3 FRACTURED VERTEBRAE.
Loosening trolley wire hangers when he felt pain in his right shoulder.
Injured was walking down passageway on the west side of the stone receiving belt hopper to throw the switch. Injured tripped on vacuum hose causing him to fall.
Employee was shocked when he came into contact with a cable on a shuttle car. He saw a doctor and was monitoned. Everything was normal with no burns so he was released.
While using a power washer to clean an oil cooler, employee slipped and the washing nozzle came in contact with the top of his left foot causing a burn & torn skin.
When the employee was visually inspecting a hoist he motioned to a fellow employee to engage the hoist to see if he could tell what the problem with the hoist was. When the hoist was engaged, a manual handle located nearby struck the employee on the left side of his hard hat protected head.
BLASTER IN CHARGE LOADED SHOT; MOVED TO A JUDGED SAFE DISTANCE TO DETONATE AND TOOK SHELTER INSIDE PARKED VEHICLE. HE DETONATED THE SHOT; A SINGLE FLYING ROCK TRAVELED IN EXCESS OF 750' STRIKING EMPLOYEE IN LEFT SIDE OF THE FACE AFTER BREAKING THROUGH THE WINDOW OF THE VEHICLE.
EMPLOYEE WAS ON 5A - BATTERY CHARGING STATION CLEANING A 636 SCOOP BATTERY. WHEN TURNING ON WASH DOWN HOSE HAD SEVERE LEAK. EMPLOYEE TRIED TO REACH SHUT OFF VALVE, LID FELL ON TOP OF RIGHT HAND - RESULTING IN AN AVULSION TO TIP OF RIGHT THUMB AND FRACTURE TO INDEX FINGER.
EMPLOYEE WAS ON THE DREDGE TIGHTENING A BOLT ON THE STONE BOX USING A CLOSED-END BOX WRENCH AND A CHEATER PIPE. HE STATES HE TIGHTENED A BOLT ONE STROKE AND THEN WHEN HE WENT TO TIGHTEN IT AGAIN THE PIPE SLIPPED AND HE FELL BACKWARDS, HITTING A PIECE OF 6"" CHANNEL IRON WITH HIS LOWER BACK AND RIGHT HIP.
EE stated he was hanging curtain in the mine when he slipped and twisted his knee. ee continued to work until 8-6-04 when he condsulted a dr.
Stepped down off the water truck and twisted left lower back/hip. Note: This incident was not reportable until lost time was incurred on 8/3/04.
INJURED WAS TRAVELING FROM 3 SECTION TO SPICK LICK IN ROVER WHEN HE COLLIDED WITH SUPPLY TRIP (BRK #16 - 1 SOUTH TRACK)
The ee was walking to the section, he tripped on the cable (for battery charger) that was lying on the bottom, causing him to fall striking his right rib cage against the charger. Note the ee's last day at work was 9-16-04.
WHILE UNKOOKING A ROPE HANGER THE MINER CABLE FELL CAUSING HIS LEFT ARM TO BE PULLED DOWN, HURTING EE SHOULDER. EE HAD CONTINUED TO WORK UNTIL 11/01/2004 AT WHICH TIME EE WAS UNABLE TO WORK.
#2 SHUTTLE CAR WAS HUNG UP AND EMPLOYEE WAS STANDING BESIDE IT. THE SHUTTLE CAR SLID OVER AND HIT HIM AND KNOCKED HIM TO THE RIB.
Employee went to grab a loading pole behind the backhoe. As he did the hoe rocked back and smashed his left hand.
EE WAS GETTING OUT OF TRUCK AND PUSHED DOWN ON DOOR HANDLE. IT BROKE OFF, LEAVING A JAGGED PIECE WHICH CUT HIS FOREARM. TEN STITCHES REQUIRED.
EMPLOYEE STATED THAT HE WAS AT THE NO.1 BATTERY CHARGING STATION CHANGING THE BATTERY ON THE NO.1 COALHAULER. AS HE STEPPED OVER THE BATTERY LIFT ARM TO DISCONNECT THE JUMPER CABLE AND CONNECT THE COALHAULER CABLE TO PLUG TO THE FRESH BATTERY, HIS FOOT SLIPPED AND HE FELT PAIN IN HIS LEFT KNEE.
EMPLOYEE WAS USING A FRONT END LOADER TO JUMP START A GENERATOR SET. STANDING ON THE LOADER ENTRY PLATFORM HE REACHED INTO THE CAB TO TILT THE SEAT FORWARD WHEN HIS FEET SLIPPED AND HE HIT HIS RIGHT SHIN AND KNEES ON THE THRESHOLD OF THE CAB DOOR. EMPLOYEE HAD MINER SCRAPE ON RIGHT SHIN. REFUSED ATTENTION AND RTW THAT MORNING. (STARTED MISSING 8-5-04 MAKING THIS REPORTABLE)
EE WAS DRIVING BACK TO THE 34 SHOVEL. SHE SLOWED DOWN TO CROSS OVER A DRAINAGE DITCH. WHEN THE TRUCK CROSSED THROUGH THE DITCH, IT SHOOK EE AND CAUSED A PAIN IN HER LEFT ELBOW. EE CONTINUED ON TO THE SHOVEL AND GOT LOADED. SHE REACHED DOWN TO GET HER WATER JUG AND COULD NOT MOVE HER ARM. DIAGNOSED WITH ELBOW STRAIN.
RIGHT SHOULDER STARTED HURTING DURING SHIFT. POSSIBLE REPETIVE MOTION INJURY. EMPLOYEE SHOVELS & GREASES EQUI9PMENT THROUGHOUT SHIFT. HAD SURGERY AND BECAME LOST TIME ON 8-26-04.
Employee was kneeling on stones cleaning around a conveyor, when a stone punctured skin on his right knee. Employee did not want to seek medical treatment at this time. About a week later employees right leg was badly swollen. He went to the doctor and was told he has cellulitis in his right leg due to an infected puncture wound on his right knee.
was putting a stone on a pallet and his back went out.
Ee was helping to take a slider tube down from an idle entry. A piece of rock fell from between the roof straps striking the slider tube, which caused the tube to forcefully strike ee.
Note: This employee is not on the payroll of Glens Falls Lehigh Cement. He is employed and paid through a Temp Agency. The employee was cleaning up at Off-spec 2 area when he slipped on clinker and fell backwards against the guard rail, fracturing ribs on left side.
Employee slipped on slick coal surface & fell, landing on his left shoulder and elbow. He bruised his shoulder.
While cutting a pipe, the piece he cut rolled over and struckhis foot.
Employee began to experience pain, in her lower back while performing her regular job duties. diagnosis: displacement of disc-surgery performed.
RIB SCREEN WAS BEING INSTALLED WHEN THE BOLTER OPERATOR DROPPED THE DRILL STEEL. EE BENT OVER TO GET STEEL AND A PIECE OF ROCK FELL OUT OF THE ROOF STRIKING HIM ON THE RT. FOOT. 20 STITCHES WERE REQUIRED TO CLOSE THE LACERATION.
Worker was inspecting mud ponds and was stung by yellow jacket.
EE WAS BACKFILLING A DRILL BLAST HOLE WHEN HE HOOKED A ROCK HIDDEN IN THE DRILL CHIPPINGS. HE FELT A PAIN IN HIS LOWER BACK THAT INCREASED LATER IN THE DAY. HE FINISHED THE SHIFT AND WENT TO THE HOSPITAL ON THE WAY HOME. HE WORKED HIS REGULAR SCHEDULE/REGULAR JOB THE 29TH AND 30TH AND TOOK ANNUAL RE-TRAINING ON THE 31ST. HE STARTED MISSING WORK 8/2/04.
Employee was putting a 6"" water line together when he felt pain in his low back. Employee has a history of back problems. Employee stated that his back would ""go out"" every so often.
EMPLOYEE WAS MOVING THE CONTINUOUS MINER ACROSS THE LAST OPEN X-CUT. WHEN HE HUNG THE MINER CABLE TO THE ROOF HIS BACK STARTED HURTING. HE WENT TO THE DOCTOR ON 7/30/04 BUT XRAYS WERE NEGATIVE. HE CONTINUED TO HAVE PAIN AND ON 9/29 HE WENT FOR A MRI. THE RESULTS SHOWED TWO BULGED DISCS AND THE DOCTOR TOLD HIM TO START MISSING WORK AND SEE A SURGEON. HE STARTED MISSING ON 9/29/04.
WHILE STEPPING ON A BUGGY HE LOST FOOTING AND FELL ONTO THE BUGGY STRIKING HIS LOWER BACK.
Ee was assisting mechanic & dragline operator in changing dragline cable. Ee claims pain in lower back, reported to foreman on 7/28. Stated it will be ok in a few days, reported to work 7/29 & 7/30. About 11:00 a.m. on 7/30 ee advised he wanted to leave early to see doctor.
EMPLOYEE WAS ON HAND AND KNEES CLEANING OUT CRUSHER CONE. WHEN HE STOOD UP, KNEE POPPED.
EE WAS DIGGING ROCK OUT FROM UNDER SCREEN PLANT. A ROCK FELL FROM THE PLANT AND HIT THE LOADER BUCKET AND BOUNCED AND HIT EE ON HIS RIGHT KNEE. CAUSED A BRUISE AND SPRAIN ON EE'S RIGHT KNEE.
Employee was rockdusting when he slipped on fallen rock and twisted his back.
EE said he was bolting top and put too much pressure on drill steel. he stated that steel sprung out and hit his rt arm and thumb.
Ee was rolling a wheel unit over to the shuttle car when lifting and pulling on unit felt pain in lower back.
EMPLOYEE WAS LIFTING UP ON BOTTOM ROLLER WHEN HE FELT PAIN IN HIS BACK.
As ee was advancing slider tube towards face he felt something pull in left shoulder.
Employee was lifting heavy brackets for 36CF (CRUSHER/FEEDER) for two shifts prior to reporting any discomfort. Employee was referred out for medical treatment and became a LWCI (LOST WORKDAY CASE INCIDENT) on 7/30/04.
STARTING ROOF BOLTER FROM OUTSIDE DECK SAID INCH TRAM LEVER WAS STUCK IN WHEN BOLTER STARTED. BOLTER STARTED TRAMMING PUSHING HIM INTO RIB.
A ROCK FELL FROM THE ROOF IN THE #2 ENTRY, TWO CROSSCUTS OUTBY THE FACE AND STRUCK EMPLOYEE IN THE HEAD, ARM AND LEG.
While bolting on the miner in 6H miner secton, an auger got stuck. When employee pulled on the stuck auger he felt pain in neck. LTA began 7-29-04.
PRY BAR HIT RT. HAND. HE WAS TURNING ROCK, WHEN IT TURNED, WHIPPED BAR FROM HAND & IT CAME BACK & HIT RT. HAND. FRACTURE RT. LONG FINGER.
EMPLOYEE REPORTED TO HIS FOREMAN AT THE BATH HOUSE AFTER HIS SHIFT, WHILE HE WAS OPERATING THE SHUTTLE CAR ON HIS SHIFT HE HIT A HOLE IN THE ROADWAY & HIS HEAD STRUCK THE ROOF OF THE CAB. NECK STRAIN.
EMPLOYEE STATES THAT WHILE BAGGING, HE TRANSFERRED A 50 POUND BAG OF BARITE FROM THE BAG SCALE TO A PALLET AND FELT A PINCHING SENSATION IN HIS LOW BACK.
INJURED WAS SERVICING A DOZER, INJURED FELL ON THE DOZER STRIKING HIS LEFT ARM RESULTING IN A BROKEN BONE IN HIS LEFT ARM.
THE INJURED WAS CHANGING THE MOTOR SHEAVE ON THE VIBRATOR. HE WAS PULLING ON THE SHEAVE TO REMOVE IT. WHEN THE SHEAVE PULLED FREE, THE INJURED LOST HIS BALANCE. THE SHEAVE FELL, MASHING THE INJURED'S FINGERS AGAINST THE MOTOR MOUNT. THE BACK OF HIS HAND WAS LACERATED NEAR THE BASE OF THE LITTLE & RING FINGERS.
EMPLOYEE WAS USING A PRY BAR, PRYING UP ON A PIECE OF ROCK ON TOP OF THE CONTINUOUS MINER. THE BAR SLIPPED AND HIS FINGER WAS SMASHED BETWEEN THE BAR AND MINE ROOF.
EE WAS MOVING SPARE RETURN ROLLER FOR CONVEYOR. A SECOND ROLLER FELL AND STRUCK EMPLOYEE ON RIGHT HAND.
EE, while crawling in low coal performing his normal duties as a cont miner operator, experienced pain in left knee. EE continued working until August 5, 2004 at which time he started missing work.
Employee got his left hand caught between the drill steel and drill pot head, injuring his left hand.
INJURED WAS ASSISTING OTHER EMPLOYEES WHILE INSTALLING A 6"" POYURETHENE FRESH WATER LINE. AS THE PIPE WAS BEING PULLED TO THE POND, THE INJURED STEPPED OVER THE PIPE. THE INJURED CAUGHT HIS RIGHT FOOT ON THE PIPE CAUSING HIM TO FALL TO THE GROUND FRACTURING HIS RIGHT FEMUR.
EMPLOYEE HAD BEEN CLEANING OUT CLAY CRUSHING MACHINE AND WAS STANDING ON THE PLATFORM AROUND THE MACHINE. HE BEGAN WALKING AND APPARENTLY MADE A MIS-STEP WITH HIS LEFT FOOT WITH RESULTING SPRAIN OF LEFT ANKLE.
WHILE DRIVING THE LOADER IN THE GRAVEL PIT, EMPLOYEE HIT A HOLE WHICH JARRED HIS HAND/ARM, JERKING HIM AROUND IN THE CAB OF THE LOADER.

1 left-5 south longwall-top bleeder entry. When pressurizing water rocprop support against roof, part of roof gave away - causing rocprop to slip hitting man in the chest.
EMPLOYEE FINISHED LOADING THE DRILL HOLES. HE WAS ATTEMPTING TO PUT UP THE UNUSED PRIMERS INTO THE PORTABLE DAY BOX. HE SAID AT THAT TIME THE WIND BLEW THE LID OF THE DAY BOX BACK ONTO HIS LEFT HAND, CATCHING HIS HAND BETWEEN THE LID AND THE DAY BOX BODY.
EMPLOYEE WAS ATTEMPTING TO CLIMB ONTO A DOZER. HE HAD ONE FOOT ON A STEP AND ONE HAND ON HANDRAIL. WHEN HE PULLED HIMSELF UP A SHARP PAIN WENT DOWN HIS LOWER BACK AND LEFT LEG CAUSING HIM TO FALL.
While operating a dozer ee ran up on a rock causing him to be jarred around in the cab. EE has suffered pain in his upper back and neck. On Aug 3rd he was seen by a E.R. physician and referred to a specialist. He was seen by the specialist on Aug 25th. He is still waiting on the specialist report.
EMPLOYEE STEPPED ON AN OLD PIECE OF BELT UNDER MUD AND WATER, SLIPPED AND TWISTED HIS RIGHT KNEE.
Employee was involved int he process of are gouging out cracks when object flew into his right eye causing them to be embedded.
Emplyee stated that while fixing a tire he hit his right index finger with a hammer.
EE WAS CLIMBING INTO A PORTAL BUS - SLIPPED AND FELL, CAUSING PAIN IN HIS LOWER BACK.
EE CLIMBED OVER HANDRAIL ON CRUSHER. HE SLIPPED, STUMBLED, THEN JUMPED, LANDING ON RIGHT ARM. RESULTED IN ELBOW FRACTURE.
Ee was exiting 992G when high winds blew debris from the bucket and boom into his eyes.
Pulling slack in dragline cable, pulled too hard.
Employee was opening the outer door on the man-material elevator when the bottom lip of the top section struck his right elbow. The outer door is in two sections that are mechanically connected so that they open together. Employee was pushing the bottom section down with his foot and the upper section struck his right elbow as it passed his arm braced on the open inside door.
Employees were lifting electrical box on crusher. Lift chain unhooked over balancing box, fell and struck employee's right lower leg, fracturing it.
Employee was dropping bottom rollers out of brakets to put shieve cable on top of rollers. Employee bent down to pick up roller and said he started feeling pain in his lower back.
Two employees were trying to take a link out of a drive chain, when a piece of metal broke off the chain link, striking an employee in the right eye, causing a deep cut on the top of the eyelid. Employee was taken to the emergency room, where he underwent surgery on his eye, and then was released from the hospital the next day.

Employee was shoveling dirt off of a Michigan 210H scraper stinger when he slipped. He tried to catch himself hitting his right wrist on the tire.
MEMBER WAS OPERATING SHUTTLE CAR WHEN HE HIT A HOLE IN ROADWAY CAUSING HIM TO BOUNCE INTO THE CANOPY INJURING HIS NECK AND BACK. THE E.R. GAVE HIM A PRESCRIPTION MEDICATION FOR HIS INJURY.
LOOSE MATERIAL ON WALKWAY SURFACE ON GROUND AROUND CRUSHING PLANT. EE SLIPPED ON A ROCK AND TWISTED RIGHT KNEE, CAUSING RIGHT KNEE LATERAL-COLLATERAL LIGAMENT SPRAIN - RIGHT KNEE MENISCAL INJURY.
EMPLOYEE REACHED OVER TO OPEN DOOR OF WATER TRUCK HIS LEFT FOOT SLIPPED, HIS WHOLE BODY FLEW INTO THE DOOR CRUSHING HIS RIGHT HAND. RIGHT RING FINGER HAD FRACTURED BONE.
EMPLOYEE WAS MOVING THE BELT AND TWISTED HIS KNEE, TEARING CARTILAGE AND SPRAINING HIS KNEE.
Ee was conducting a pre-shift inspection on the dozer. It was raining and ee was standing on the track and he slipped and fell hitting his left shoulder on the ground.
While moving and twisting a vibrating pan into position employee felt a sharp pain in his back.
THE CONTINUOUS MOTION OF MOVING 50 LB. BAGS FROM ONE PALLET TO SCALES THEN TO ANOTHER PALLET CAUSED MUSCLE STRAIN TO LOWER BACK.
EE STATED HE WAS TRYING TO UNPLUG THE HEAD ON THE ROOF BOLTER. HE PLACED HIS HAND ON THE SLIDE AND HIS COWORKER WAS TO ACTIVATE THE ROTATION LEVER BUT HIT THE WRONG LEVER AND DROPPED THE HEAD, CATCHING EE'S HAND, CAUSING A PINCH TO EE'S THUMB.
After fueling the 16g grader, the ee was dismounting the 16g with associated fuel hose. Due to stiffness of the hose, it twisted causing the ee to fall. EE landed on his right hand jamming his elbow.
Employee states that he was in process of unloading shovel bucket pins from the back end of his service truck. As he was rolling or scooting the pin from his truck, after it landed on the bumper, it either bounced or flipped and struck his right hand. Three of his fingers were mashed with the index finger being fractured.
The employee de-railed the mantrip and was using a jack and bar to put the mantrip back on the rail. while using the jack, the handle slipped out of his hand and struck the ee's mouth. the ee's front teeth were damaged and dental work was needed.
EMPLOYEE WAS CUTTING STEEL. A PIECE OF SLAG GOT INTO HIS EYE.

6H section, while walking he stepped on a flat rock, slipped and fell twisting his left knee and fell onto both knees.

Employee was working on pump cable at #4 power center. When put breaker back in, it arced burning his hand.
ALLEGEDLY EE WAS PULLING ON PIPE WRENCH TO LOOSEN FILTER ON LOADER WHEN HE FELT PAIN IN HIS LT.HIP/ BACK AREA. THIS RESULTED IN A DIAGNOSIS OF LOW BACK PAIN W/SCIATIC NERVE INVOLVEMENT. THIS INJURY WAS NOT REPORTED UNTIL 8/2/04. IT IS UNDER INVESTIGATION.
Employee was using an endloader to move a large rock from the dig face. A section of the rock broke loose, causing the endloader to jar the employee resulting in a strain to the neck and shoulder.
EE was riding mantrip with mandoor positioned on top of mantrip. While tramming through an intersection mandoor caught the coal rib and was pushed toward back of mantrip. the mandoor caught ee in right leg mashing it between mandoor and mantrip.

EE EXPERIENCED A BURNING SENSATION IN HIS RIGHT SHOULDER. IT DID NOT GO AWAY AND IS ANNOYING. EE CLAIMS POOR LIGHTING WAS A FACTOR IN RECEIVING THE INJURY. LOST TIME STARTS ON 5/4/05.

Employee was trying to pull the wheels out from the stacking conveyor. He was pushing quite hard on the wheels when he felt a ""pop"" in his right knee.
Victim removed the radiator cap from a D-9 dozer while engine was still hot. The hot anti-freeze blew out and hit him on the forearms, chest and stomach resulting in 1st and 2nd degree burns.
EE WAS WORKING ON D9L #1222 DOZER. HE WAS GOING TO PUT A DIPSTICK TUBE UNDER THE FLOOR OF THE DOZER. HE TOOK THE FLOOR BOARD OUT OF THE CAB, CLOSED THE DOOR, GOT DOWN OFF THE DOZER TO GET WRENCH AND NEW PART, GOT BACK UP ON THE DOZER. HE OPENED THE DOOR AND STEPPED IN FALLING THROUGH THE HOLE IN THE FLOOR. BRUISED LEFT LUNG AND HAIR LINE FRACTURE OF RIB.
MEMBER WAS EXITING AN S&S RAIL ROVER ON CHARGE. AS HE WAS CLIMBING OUT OF THE DECK HE SLIPPED AND FELL BACKWARD LANDING ON HIS LEFT SHOULDER. HE DID NOT START MISSING WORK UNTIL 8/26/04.
DID NOT START LOSING TIME UNTIL 3/29/05. While going through doors in brattice, a door closed on her shoulder. She has been going to the doctor since the date of accident but is now losing time for surgery.
While operating a man-bus, lost control of it and jumped from the bus. Abrasions and contusions of face and wrists and complaining of low back pain.
Employee was operating D-11 dozer and track slipped off large rock causing dozer to slide into a hole, which resulted in a jar to the ee. The result was pain in lower back.
EE was in the process of prying down a loose coal rib with a prybar when additional rib also fell just out by the place he was prying, resulting in the material striking him on his left leg.
EE was working on a rockbuster #2514 when a low back sprain occurred.
EE states that he twisted his right knee when he slipped on a pile of coal that was spilled on a stairway. EE had his knee examined by an orthopedic physician, and was initially diagnosed with a sprain and released o full work duties. Re-examined on 8-13-04 when he was diagnosed with a lateral tear of the meniscus.

CLIMBING LADDER TO GET IN DOZER, SLIPPED ON LADDER & FELT PULL IN LEFT ARM WHEN HE HELD ON LADDER. REPORTED INJURY ON 7/22/04 FIRST SOUGHT MEDICAL TREATMENT ON 1/12/05. REASON FOR REPORTING: MEDICAL TREATMENT.
OPERATING CONTINUOUS MINER IN COAL FACE, MAKING THE RIGHT SIDE CUT THE MACHINE HAD PENETRATED APPROXIMATELY 15' DEEP WHEN HE OBSERVED SPARKS NEAR THE MINE ROOF. SUDDENLY PAIN WAS FELT TO HIS LEFT ARM. THE CUTTING BITS HAD CONTACTED A SULFUR BALL NEAR THE MINE ROOF. A SLIVER OF STEEL EITHER OFF A DRUM BIT HOLDER OR CUTTING BIT BROKE AWAY, FLYING BACK AND STRIKING HIS LEFT FOREARM.
Ee was helping hang the miner cable. Miner operator trammed the miner forward while they were hanging cable and it pulled too tight causing it to fall while ee had a hold of it. Bent his wrist back.
EMPLOYEE WAS EXTRACTING COAL WITH THE GRADALL IN THE CORNER AREA OF THE PIT WHEN THE HIGH WALL SUDDENLY GAVE WAY COMING DOWN ON PART OF THE GRADALL.
EMPLOYEE WAS INSTALLING A BOLT IN ROOF WHEN BOLT ROTATED AND STRUCK HIM ON THE LEFT WRIST. MISSED WORK ON 7/22/04 BECAUSE AVAILABILITY OF DR. TO CAST ARM.
Employee and co-worker carried a bottle of oxygen approximately 25 feet and loaded it into a service truck, a lift of approximately 3 feet. During the lift he felt a burning in the lower right side of his back.

EE slipped and fell during lock out/tag out procedure on an excavator. He was on the catwalk above the tracks when his foot slipped off and he fell to the dirt below (he said his boots were wet). He broke his left hip and femur.
EE SAW 992C (WHEEL LOADER) ON FIRE AND RAN TO HELP EXTINGUISH THE FIRE WHEN HE INHALED CHEMICALS FROM THE FIRE EXTINGUISHER.
EMPLOYEE WAS PUTTING SUPPLIES IN SCOOP WHEN A ROCK FELL IN BETWEEN BOLTS HITTING HIM ON THE HEAD. HE LOST CONSCIOUSNESS FOR ABOUT 1-5 MINUTES. THERE WERE NO VISIBLE INJURIES.
INJURED WAS OPERATING MINER WITH REMOTE WHEN PIECE OF ROCK FELL BETWEEN BOLT AND RIB, STRIKING INJURED ON BACK AND NECK.
EMPLOYEE WAS REMOVING A BEARING ON CONVEYOR BELT. HE WAS USING A HAMMER AND PUNCH. THE POINT BROKE OFF AND THE PUNCH HIT HIS EYE.
INJ WAS INSTALLING J30 BIT INSERTS IN CUTTER DRUM USING HAMMER WHEN A PIECE OF METAL SEPERATED FROM THE INSERT STRIKING INJ IN THROAT.
Employee stepped up on side of dump truck to talk to driver and took hold of hot exhaust pipe.
Employee was split tramming single head roofbolter when he slew the machine around it bumped him on the left hip.
EMPLOYEE WAS USING A SLEDGE HAMMER TO REMOVE A PIN FROM A SCOOP JACK. WHILE PERFORMING THIS TASK HE FELT PAIN IN HIS BACK. HE REPORTED THE INCIDENT, & A FIRST REPORT OF INJURY WAS GENERATED. THE PAIN SUBSIDED, HE ELECTED NOT TO SEE A DR. EMPLOYEE CONTINUED TO WORK UNTIL 8/9/04 WHEN THE PAIN RETURNED, HE DECIDED TO SEE A DR. WHO TOOK HIM OFF WORK.
INJURED PLACED RIGHT HAND ON TRS FRAME TO HELP PULL HIMSELF UP INTO A WALK THROUGH BOLTER. HE HEARD ROCK ABOVE HIS HEAD START TO BREAK LOOSE FROM ROOF. HE STEPPED BACK AS THE ROCK FELL, THE ROCK GLAZED HIS RIGHT SCALP BUT CAUGHT HIS RIGHT HAND AGAINST FRAME.
Person was trying to lift belt skirting out at belt discharge, his foot slipped as he was lifting and he felt pain in lower left part of his back. belts were idle when accident occurred.
Employee fell from the bumper of his service truck. He was attempting to raise the hood and was using the grill section as a grab handle. The grill section broke away, causing him to loose his balance and fall onto the concrete shop floor. He has contusions to his lower back and left arm.
As ee was installing a cable bolt he accidentally hit the control levers with a bolt causing the drill head to swing out against his left foot, causing a fracture.
EMPLOYEE WAS CLEARING THE SPACE IN THE QUARRY TO SET DOWN THE GROUT BOX WHEN IT HIT HIM IN THE ARM AND NECK.
While walking down steps of the Pioneer Primary Crusher EE slipped. Foot came out from under him. caught himself but his leg fell on the step cutting leg.
Employee was moving in and around a litter fence and tripped over a section of the fence. he fell backwards onto the piping. An ambulance was called where he was taken to the hospital and treated for three broken ribs and a collapsed lung.
Tightening bolts on Bohringer - pulled back muscle.
EMPLOYEE WAS HANGING VENTILATION FAN WHEN FAN SLIPPED OUT OF HANGER. FAN CAME OUT OF FRONT HANGER WHILE TIGHTENING JUNCTION BOX TO SIDE OF FAN. WHEN THE FAN SLIPPED COMING OUT OF FRONT HANGER IT TIPPED (FRONT) DOWN STRIKING EMPLOYEE IN CHEST/RIB AREA. EE WAS TRANSPORTED BY AIR TO HOSPITAL.
EE was steam cleaning a 980 end loader, pulled the hose from off the steam cleaner, and bumped his right elbow on the end loader counterweight. he sought medical attention and thing broken. he continued to work until july 27th when he began misisng work. he was admitted to the hospital due to welling having to have his elbow drained.
Ee was operating a water truck to fill decant pipe. he was backing water touch up to fill decant pipe, back truck off road into ditch and turned loaded water truck onto side.
Employee was riding in a loader bucket, bucket bounced when it rubbed rib, and he was holding on to pivot of injuector bucket smashing his finger tips of his left hand.
Employee stated that he was removing the right side top bucket pin from the loader bucket. There was pressure on the pin and when he struck the pin with a hammer, the pin jumped out, fell and landed on top of the employee's right foot.
WHILE OPERATING 988B LOADER THE CONTINUOUS BOUNCING UP AND DOWN HURT HIS LEFT SHOULDER AND NECK AREA AND THE LOOKING BACK OVER HIS SHOULDER REPEATEDLY.
A rib rolled out striking injured in both legs and lower back area.
EMPLOYEE CLIMBED ONTO THE DOZER ON TRACKS, LOST BALANCE AND FELL OFF DOZER.
EMPLOYEE REPORTED SEVERE BACK PAIN WHILE OPERATING A BULLDOZER.
EE was helping other take guard of feeder to replace a belt. when he took the last bolt out the guard fell and mashed his finger on the tailpiece clipped the meet off the tip of his ring finger left hand.
Late entry. Employee was helping unload three inch pipe from a truck . During the course of unloading the pipes, employee felt a pain in his back. Did not see doctor until next day. Returned to doctor two days later and doctor placed him on disability leave. Employee missed twenty nine days of work. Internal audit of books discovered that accident had not been reported.
EE WAS TRAVELING IN A WATER WAGON TOWARDS CARTER #2 MINE AND WAS TRYING TO SLOW DOWN. THE BRAKES LOCKEC UP AND THE TRUCK ROLLED OVER.
Cleaning the dredge with a 2"" water hose, he went back to untangle the hose and stepped on a 2"" step and then stepped on the hose. This caused him to roll his left ankle. Diagnosed with a strain, given prescription, crutches, and taken off work.
Employee was clearing dirt and rocks from around a pipe trench on uneven ground, Employee lost his balance and fell sideways, his foot was between pipe and dirt, his weight of body falling sideways caused a fracture to his right ankle.

WHILE PULLING CABLE FROM CONTINUOUS MINING MACHINE OUT OF ROADWAY, EE COMPLAINED OF BACK PAIN. IT APPEARED THAT HE STRAINED HIS BACK.
Employee dismounted her forklift. She was aware of a second forklift backing out of the warehouse and the driver was also aware of the employee dismounting her forklift. The employee began walking toward the moving forklift. The driver lost sight of her, assuming she had walking around the back of her vehicle. The driver moved forward and ran over the employee's right foot.
Employee climbing off dozer, slipped and fell approx. 18"" into a sand refuse pile, landing on his back, climbed back onto dozer and operatoed for approx. 30 minutes then complained of numbness in his feet
Employee failed to recognize the saw block tipping over, which was very out of normal. The method used was the same as all routine extractions. He was able to distance himself enough to avoid serious injury, but was hit by the drill and dogs, knocking him down and causing hip fracture and arm lacerations.

THE THOUGHT EE HAD GONE TO RESTROOM. WHEN THEY JOGGED THE BELT, THE EE GOT HIS 4 LEFT FINGERS IN BETWEEN PULLEYS AND BELT AT DRIVE MOTOR.

The ee was helping clean the top off the miner and was using a pry bar to remove a rock. he finished his shift but later informed the foreman that there was pain in his right shoulder. the ex-ray was negative, muscle pull to right shoulder.

EMPLOYEE CLAIMS TO HAVE INJURED HIS BACK ATTEMPTING TO MOVE A LARGE ROCK FROM THE HAUL ROAD.

EE was assisting in aligning used truck tires so they could be lifted into a dump trailer with a forklift. The ee stated that he pulled something in his back. EE returned to work on 7-19-04, worked 4 hours, went home.
Employee was taken to Kaiser Permanente on 7/17/04 and was taken off work. High-pressure water hose was tangled in the head pulley and gravity take-up snubber pulley, causing it to pull employee's right arm between idler and the moving conveyor belt resulting in two compound fracturers.

Ee was in the process of installing roof bolts in face of #4 entry. Drill steel that he was using to drill roof bolt hole hung on top of drill pot as he was lowering the steel out of hole. He stated he was holding steel w/rt hand & was moving drill pot back & forth to free the steel. When steel came loose it caught little finger of rt hand between pot & steel.
Became reportable on 8-10-04. The ee was re-positioning a stuck rock in the 6-North conveyer belt feeder when he experienced pain in neck and roght shoulder. He continued working until 8-10-04 at which time he went to Dr and had MRI which revealed the cause of pain.

Worker was using a ""mushroomed"" wedge to split a quarry block. When the worker struck the wedge a piece of metal from the wedge struck the worker on the upper forearm just above the inner elbow area causing a laceration which required 3 stitches to close.

Injured was removing a bag of tools from the back of a three wheeler personnel carrier during shift change when an individual from the oncoming shift boarded the machine and trammed it in reverse. The machine ran over the injured's foot, knocked him down, and ran onto his leg. The operator thought everyone was clear of the three wheeler
Went to unplug #3 screw w/air hose, was holding himself up w/left hand, operating air hose w/right. Slipped & saw his hand slip into screw, at the same time I pulled the trip cord but not quick enough. When he pulled his hand out I realized my fingertip was missing. The screw was guarded & had trip wire, small opening for maintenance only, to view for proper operation.
The employee was climbing the hill of the pit in a haul truck to dump rubbish at the grout pile. He tried to downshift on an incline and lost momentum to climb the hill. The truck rolled back to the quarry floor and slipped off the road causing it to tip on it's side. The employee jumped from the truck and sprained or fractured his wrist and had minor skin scrapes.

EMPLOYEE WAS HELPING UNLOAD TIMBERS & FELT PAIN IN RT. SHOULDER. DID NOT THINK IT WAS HURT VERY BAD. EE HAS CONTINUED TO WORK UNTIL 8/3/04 UNTIL DR. HAS TOLD HIM THAT HE MUST OPERATE ON SHOULDER TO DETERMINE WHY HE CONTINUES TO HAVE PAIN & WHY HE CANNOT LIFT HIS RT. ARM.
Employee was operating roof bolter and a piece of draw rock fell striking the employee on the lower back.
Twisted back while dismantling and moving crusher equipment. Alledgedly happened on 7/15 but employee worked 7/16 & 7/17 then called off work on Mon. 7/19 and said he was going to the chiropractor. He was observed moving gingerly on 7/16 and when plant manager asked what happened he said he thought he may have twisted his back on 7/15 but that he would be ok.
EMPLOYEE WAS WORKING ON THE WATER TRUCK WHEN HE GOT HIS FINGER MASHED WITH A HAMMER. HE CONTINUED TO WORK UNTIL 7-26-04 WHEN HE WAS PLACED UNABLE TO RETURN TO WORK. HIS FINGER IS BROKEN AND HAD TO BE RESET.
EE WAS GREASING. ANOTHER WORKER TURNED ON THE CONVEYOR AND CAUGHT EE'S LEFT FOOT IN BELT BETWEEN WIPER AND BELT. EE HURT HIS LEFT FOOT, LEG & ANKLE.
THE EMPLOYEE WAS CHANGING TRAM HOSES ON A STAMLER FEEDER WHEN HE FELT A PAIN IN HIS LOWER BACK. HE STARTED MISSING WORK ON 7/21/04. RULED AS OCCUPATIONAL INJURY ON 9/9/04 WVA WORKER'S COMP.
THE EMPLOYEE STEPPED FROM A BELT BED RAIL TO THE MINE FLOOR AND AT THE TIME HE DID NOT FEEL A TWIST OR PAIN IN HIS RIGHT ANKLE. HE FINISHED THAT SHIFT AND HIS FOLLOWING SHIFT. THE SWELLING IN HIS RIGHT ANKLE DID NOT GO DOWN AND HE DECIDED AN EXAMINATION WAS NEEDED. THE X-RAY SHOWED A HAIRLINE FRACTURE IN HIS RIGHT FOOT. HIS FIRST DAY AWAY FROM WORK WAS 7/19/04.

THE EMPLOYEE SAID THAT HE WAS CUTTING AND LOADING COAL WITH THE CONTINUOUS MINER WHEN THE COAL RIB ROLLED AND STRUCK HIM IN THE UPPER BACK.
While walking alongside miner to help roof bolter a faceman put his hand on machine for balance. a 12""x8""x2"" piece of rock fell and hit him on right hand.
Ee stated that he was attempting to straighten a piece of roof bolt steel with the head of the bolt machine when the steel rolled away from him and tore the finger tip and nail away from his finger. He was transportal to the hosptial where he was x rayed and stitches were applied to reattach the finger tip. the x rays showed no fracture.
EMPLOYEE HIT ELBOW AGAINST BIT TRAY ON ROOF BOLTER. HE HAS BEEN TAKEN OFF WORK FOR TERAPY FOR THE TENDON IN HIS RIGHT ELBOW. EMPLOYEE STARTED MISSING WORK 8/10/04.
Employee stated he was walking to uncouple supply cars and felt apain in left knee.
Employee states he was climbing ladder of skip loader when his foot slipped on the top step causing him to fall to the ground.
EE WAS WORKING WITH OTHER EMPLOYEES TO CUT THE BEARING OFF THE SHAFT ON THE #2 CONVEYOR. THEY HAD HUNG A TORCH ON THE I-BEAM OVERHEAD FOR LIGHT. WHILE THEY WERE PUTTING A NEW BEARING ON, THE TORCH FELL AND STRUCK INJURED, KNOCKING OFF HIS HARD HAT AND SAFETY GLASSES. THE TORCH STRUCK HIS RIGHT EYE.
Employee claims back injury related to loading scrap crusher hammers.
BASED UPON THE INFORMATION RECEIVED AT THIS TIME, EE WAS PUTTING A SPRING ON EE'S TRUCK AND HE FELL OFF AND HIT HIS RIGHT ARM AND IT STARTED TO SWELL. THE DR SAID THAT HE BRUISED A MUSCLE AND WOULD BE BACK TO WORK ON THE FOLLOWING MONDAY. EE WENT BACK TO THE DR. ON MONDAY AND CALLED AND SAID THAT HE WOULD BE OFF LONGER THAN ORIGINALLY ANTICIPATED
EMPLOYEE WAS EXITING MANTRIP AND FELL AND HURT RT. ANKLE. WENT TO HOSPITAL MORNING OF 7-15-03 AND HAD X-RAYS AND HAD A FRACTURED RT. ANKLE.
6H section, helping make splice on belt and adding a piece of belt, as he was lifting and pulling piece of belt into place and pulled a muscle in his groin area. diagnosised as hernia lost work days began 7-26-04.
BOLTER OPERATOR PULLED INTO THE CUT, SET HIS TRS AND PREPARED TO DRILL HIS FIRST BOLT HOLE WHEN A PIECE OF ROOF ROCK ABOUT 100 LBS. FELL FROM THE ROOF, CATCHING HIM IN THE BACK OF THE NECK AND SLIDING DOWN HIS ARM HITTING HIS HAND AGAINST THE BOLTER.
Employee was changing fuse in electrical box at palnt, when he went to put in new fuse it arced causing 1st and 2nd degree burns to his hands and arms.
EMPLOYEE STATED THAT A PIECE OF DRAW ROCK BETWEEN THE RIB BOLT AND RIB FELL AND STRUCK HIM ON THE RIGHT LEG. HE WAS X-RAYED AND IT WAS DETERMINED THAT HE HAD SUFFERED A BROKEN KNEE CAP.
EMPLOYEE GRABBED A ROCK TO GET IT OFF THE FEEDER WHILE THE FEEDER WAS RUNNING. THE ROCK KICKED UP & FOULED HIS HAND AGAINST THE ROOF, BRUISING HIS HAND AND CUTTING HIS FINGERS.
INJURED PERSON WAS PLACING A PIECE OF MINE BELT ON BOTTOM IN FRONT OF A POWER CENTER WHEN HE INADVERTENTLY STEPPED ON A NAIL STICKING UP OUT OF BELT. HE SUSTAINED A PUNCTURE WOUND OF LEFT FOOT WHICH REQUIRED HIM TO LOSE TIME FROM WORK.
Employee was ascending a haul truck ladder when his right knee popped. He did not report the incident until 7-16-04. The employee did not seek medical treatment other than first aid until 7-29-04. This report is being filed based on the medical treatment date of 7-29-04.
This employee was taken to the emergency room on 07/14/04 with complaints of pain in his leg and foot. After X-rays and scans of his lower region the doctor found that he has ruptured discs in his back. He could not pinpoint an exact date as to when the injury was incurred nor could he attribute the injury specifically to any one work-related task.
He was walking underground and twisted right knee.
