While trying to open a void on the #5 clean coal stockpile, employee while operating a Caterpillar D9 dozer fell into the void. Recovery was successful with no injuries.
An unintentional roof fall occured in the #6 entry of South West Mains, spad 7557, which fell on a section of the 6 #1 belt causing it to be shut down. Dimensions: 50' long x 17 feet wide x 5'6"" high. Support: 5' fully grouted tension rebar.

FALL WAS APPROX. 90' X 80' X 7' THICK AND WILL NOT BE CLEANED UP. FALL IS APPROX. 2,525' FROM FACE OF #001 SECTION. THIS FALL WAS REPORTED BY FIRE BOSS.
A roof fall occurred on the 2L4W-2L panel in entry #6, cross-cut 8. The cause of the fall is weak laminated roof shale commonly refered to as stack rock. The fall occurred in the entry adjacent to the primary escapeway for #3 unit.
A roof fall in the 1 South x-cut #29 in the #3 entry measuring 35 ft. x 18 ft. x 6 ft. - 9 ft. The area was mined in April 2004 and supported with 6 ft. tension rebar bolts and 9 ft. mega bolts. This fall was caused by a hidden slip in the mine roof.
When mining the xcut 5 to 6 in 1 R/T, the Roof Bolters detected a 1 foot break - they added extra 5' bolts, when the miner started the xcut 6 to 7, knowing that there was a break at 1 foot only took a 15' lift when the lift was completed approximately 2"" of rock fell, roof started working and caved approximately 6' high - 18' wide - 28' long.
AT APPROX 11:10 AM SMOKE WAS DISCOVERED IN THE AREA JUST OUTSIDE OF THE SUGAR RUN BOTTOM DUMPSTERS OFFICE. UPON IMMEDIATE INVESTIGATION FLAMES WERE OBSERVED COMING OUT OF THE TOP OF #33 SEAL.

ROOF FALL IN THE HIGH QUALITY 4 EAST GATE ENTRIES, #4 ENTRY FROM 45+50' TO APPROX. 46+10'. FALL IS APPROX. 60 FT LONG, 18 FT WIDE, 12 FT HIGH. AREA MINED FEB/MARCH 2004 WITH CONTINUOUS MINER AND SUPPORTED WITH 7 FT POINT ANCHOR ROOF BOLTS PLUS 6 - 100 TON BIG JOHN PROPS. THE AREA HAD A CLAY VEIN RUNNING DIAGONALLY THROUGH IT.
There was an ignition on the 3-north right section at the face between 7 and 8 entry at 118 crosscut. the ignition lasted 30 seconds and was extinguished with two wash down hoses and the sprays of the miner. there were no injuries and no damage to equipment
the shearer operator reported seeing two orange and blue flames about the size of a gallon jug come out of the top and go down the panline lasting for 2-3 seconds as the shearer approached 107 shield mining from the tail to the head. the lone observe said the flames were put out by the drum sprays and the spray bar on the shearer.
CONTINUOUS MINER WAS STUCK IN PILLAR LINE & THEN SOME ROCK FELL ON MINER. MINER RETRIEVED ON 6/29/04 AT APPROX. 12:00PM.
Shoal Creek Mine experienced an unplanned roof fall on the B-11 section in the No.3 entry last open crosscut. The Fletcher drill was in by the roof fall and was blocked. This was above anchorage of the bolts.

friction caused a methane ignition on the longwall panel 49 face. when the shearer contacted a sulfur ball in the face as the shearer approached 149 shield the operator observed a blue and orange flame about 1.5 cubic foot wrap around the headgate drum following the bits into the panline. a lone observer said duration of the flame was 2-3 second.

While mining Rooms 22,23, 24, 25 - we were in our last set of xcuts, a xcut was turned 23 to 22 Room - when testing the roof, it was drummy and a short cut was taken 15' - when the bolter came into bolt, they heard the roof cracking and working. Area did fall in a approx. 12:15 p.m.

DURING HOISTING AT VERY SLOW SPEED, THE HOISTMAN NOTICED A DELAY IN THE APPLICATION OF THE HOIST'S BRAKING SYSTEM. NO INJURY OCCURRED AS MEN WERE NOT ON BOARD THE CONVEYANCE. MSHA WAS NOTIFIED IMMEDIATELY. THE CONVEYANCE AND HOIST HAVE BEEN SHUT DOWN UNTIL REPAIRED.
Roof fall out by #002 section in #6 entry in intersection toward #7 entry, fall was approx 30 ft long by 20 ft wide and 6 ft thick. Top was bolted with 5 ft resin bolts. Fall was 2 brks in by 7-N head. Fall was 1 brk out by spad 28323 and 3 brk in by spad 28316. Fall will be cleaned up and is 1800 ft from face.
2 TIRES THAT HAD BEEN DISPOSED OF 100 FT DOWN THE SPOIL SLOPE OF PINON PIT CAUGHT FIRE. NO ONE WAS WORKING IN PIT AT THE TIME. WATER TRUCK EXTINGUISHED TIRES WITHIN 35 MINS. MSHA NOTIFIED. PINON RAMP 3 IS A NORMAL TIRE DISPOSAL AREA (MOST TIRES AT BOTTOM OF PIT) AND AN ACTIVE ASH DISPOSAL PIT. DUMP ELEVATION TO BE COMPLETED BY END OF AUGUST, 2004.
IT APPEARS THAT ONE OF THE #1 COAL MILL ROLLS LOST THE OIL THAT LUBRICATES IT. THIS ALLOWED THE ROLL TO BECOME VERY HOT WHICH THEN PROVIDED AN IGNITION SOURCE FOR THE GROUND COAL. THE IGNITION OF COAL CAUSED THE RUPTURE DISCS IN THE COAL MILL DUST COLLECTOR TO BLOW OUT AS THEY WERE DESIGNED TO DO.

At 5:30 am mine examiner discovered a roof fall in the roon neck of the 2nd southwest butt section. Fall was outby station #3850 by approx 50'. Roof fall was 25' long, 10' to 20' wide, 4' to 6' high. Area was bolted with 4' fully grout rods with 8"" oversize bearing plates. The rock in the fall area looks like soap stone with slick and slide.

WHILE TRAVELING WITH AN INSPECTOR CHECKING EP'S, THEY FOUND A FALL AT EP 31. IT WAS 20 FT X 20 FT X 4 FT THICK. 1 BREAK INBY SPAD. THIS AREA HAD 4 FT RESIN BOLTS - AREA CONSISTS OF STACK SHALE.
Unintentional roof fall between #4 & #4 entry 2EP 5820' mark. 18'L x 18'W x 6.5'H. Not in intersection, but inby tail MMU 050. Bolted with 6TR bolts.
Hot lime from Kiln 5 cooler pocket dropped onto cooler conveyor causing ignition of the rubber conveyor. Fire caused damage to cooler conveyor and to misc. electrical wiring in the area.
A failure in the hanging wall at 32W 3700 occurred between 6/21/04 dayshift & 6/22/04 dayshift that appears to meet MSHA's reporting criteria. No one working in the area on night shift. The heading has been open for appx. 10 months & was cable bolted about 2 months after initial excavation. The failed section measured approximately 10' h x 6' d x 20' l.

A ROOF FALL OCCURRED AT THE INTERSECTION OF 88 XCUT IN THE #6 ENTRY INBY THE BELT TAIL. THE FALL WAS 18' X 18' X 8' HIGH. THE AREA IS SUPPORTED BY 5' FULLY GROUTED RESIN BOLTS AND 10' POINT ANCHOR BOLTS.
Employee was traming a 690 drill up a bench road when he noticed the machine loosing power. He looked in his right side mirror and saw flames coming out of the right engine compartment. He shut down the machine and exited it. Employee obtained the machines fire extingushers and used them on the fire. The fire continued to grow engulfing the machine.

AN UNPLANNED ROOF FALL OCCURRED IN THE #4 ENTRY AT BRK 12 & 14. 4'-10' HIGH, 20' WIDE, 20' LONG. LOCATION SPAD #1597.
Cutting with oxygen and aceteyline torches was being conducted on the tailgate cowl at shield 20, when a spark or slag igniteda bleeder in the floor under the shunck breaker.
ROOF FALL X18 6NP TRAVELWAY. 35'L X 20'W X 8-10'H 4WAY INTERSECTION. WILL NOT BE CLEANED UP. BOLTED WITH 6'X 8' TR BOLTS.
AFTER MAN TRANSFER IN THE CONVEYANCE, THE HOISTMAN FAILED TO HAVE THE CONVEYANCE RECONFIGURED TO MOVE MUCK. ONCE LOADED WITH MUCK, THE CONVEYANCE DUMP DOOR BECAME JAMMED. HOISTING OPERATIONS CEASED UNTIL THE JAM WAS CLEARED. NO DAMAGE TO EQUIPMENT OR INJURY TO PEOPLE AS A RESULT OF THIS INCIDENT.
A roof fall of approximately 30'L x 15'W x 6'H dimensions occurred 11,200' outby face, ss #1766, #6 entry. Intake primary escapeway. No injury or damage to equipment occurred. The fall will be cleaned up.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON 6 RIGHT SECTION, ONE BREAK INBY SPAD #22344 IN THE #2 ENTRY. THE APPROXIMATE DIMENSIONS OF THE ROCK FALL AREA ARE 10+ FT X 30FT X 50FT.
Three major electrical faults occurred simultaneously: flashing a hoist motor, blowing two high voltage fuses, and arcing an inverter SCR cell. These events ignited dust which burned the paper filters in an A/C unit and started small fires in several locations. All fires were readily extinguished. No ignition damage except the A/C filters. No person was present.
UNINTENTIONAL ROOF FALL #11 ENTRY 2 EP AT 5940' MARK. 25' L X 20' W X 8-10' H. BOLTED WITH 8' TR BOLTS. MINED 6-15-04, 3 WAY INTERSECTION WILL NOT BE CLEANED UP.
At 5:30 pm an ignition of coal dust occurred within the #3 kiln coal firing system causing the pressure relief vent to burst. No personal injuries or property damage resulted from this occurrance.
A round was shot in 209 stope and a wedge fell from the back when the ground supporting it was shot. The section of ground that failed was a remnant pillar nose left over from mining the 224 lift above. The pillar nose was bounded on three sides by cemented tailings backfill and was 12'Lx15'Wx10'D, beyond length of 6' rockbolts.
#7 ENTRY BETWEEN CROSS CUT #18 & #19 ON #3 UNIT. NEAR FAULT AREA IN 2ND WEST PANEL. 70' LONG X 19' WIDE X 2' - 8' HIGH.
No injuries, equipment damage, illnesses. In the 18E by 355 intersection, there were no miners in the area, there was no equipment in the area, it did not impede passage of effect ventilation. size of roof fall approx 90' by 45' by 11' deep.
At approximately 6am the #3 hoist kicked out at the 1900' level. The back-up generator was started and the miner's were lowered the rest of the way to the bottom. Upon investigation it wa determined the the drive unit failed because 2 SCR's and the firing controllers they plug into, failed. Replaced - functional.
A ROOF FALL OCCURRED ON WEST MAIN DEVELOPMENT SECTION, 130'OUTBY THE FACE IN #3 INTERSECTION AT SPAD 73+15. THIS AREA HAD PREVIOUSLY BEEN PINNED WITH 6'POINT ANCHOR RESIN BOLTS. THE AREA IS APPROX. 9'HIGH & 29'WIDE & 33'LONG.
On 6 North section, No. 3 entry, last intersection at survey spad #9186, an un-intentional roof fall occurred, approx 16' by 20' by up to 8' high. Fell just at anchorage of 8' combination/ resin roof bolts.
2nd 48 beltline, x-cut 58, no. 4 entry. Length 16', width 16', thickness 4'.

HOIST WAS BEING OPERATED WITH 3 MEN DOING MAINTENANCE WITHIN THE SHAFT. HEAVY RAIN WAS FALLING. RUNOFF EVENTUALLY OVERCAME THE DITCHING AROUND BUILDINGS. WATER FLOODED INTO A STORAGE BLDG., INTO AN OLD CABLEWAY IN THE FLOOR, THROUGH CONDUIT INTO AN ACTIVE MANHOLE, THROUGH CONDUIT INTO THE HOIST BLDG. & FLOODED THE BASEMENT PIT.
Unit #3 no. 3 entry, slip running across unit. Length 22', width 18', thickness 8'.
DURING THE 1ST SHIFT ON 6/17/04, THE ROOF STARTED WORKING AT SPAD #1297 IN THE #3 HEADING. EMPLOYEES WERE EVACUATED FROM THE AFFECTED AREA AND THE AREA WAS DANGERED OFF. THERE WERE NO INJURIES AND NO EQUIPMENT WAS INVOLVED. THE FALL WAS 8' HIGH, 30' LONG, AND 30' WIDE AT THE INTERSECTION. THE FALL HAS BEEN CLEANED UP AND BOLTED.
A ROOF FALL OCCURRED IN THE #1 AND #2 ENTRIES OF THE APPROVED BLEEDER SYSTEM. THE FALL WAS TO THE RIGHT OF AND INBY SURVEY STATIONS #21665 AND #21666. THE FALL WAS APPROX 18' WIDE, UNKNOWN HEIGHT, AND UNKNOWN LENGTH. THIS FALL BLOCKED THE TWO TRAVELWAYS OF THE BLEEDER SYSTEM.
Brake pad on the production hoist brake caliper came loose and a portion of the pad broke loose. To make repairs the production hoist was taken out of service and the mine was evacuted. Hoist was tested and returned to sevice at 2:45 AM No injuries.
While hoisting, hoistman heard odd noise. He gave visual inspection, noticed rope slack on North drum, N. skip was hung. Replaced worn skip, mine evacuated. Down 3:19 a.m. to 6:20 a.m.
A roof fall was discovered in the #7 entry at spad #9751. The roof fall measured 80' long by 18' wide by 6' high.
Damage to hoisting facility, supply cage at Wana facility, impact of cage hitting the bottom, damaged the cages floor and side walls requiring repair work. Out of service until repaired.
Methane liberation in excess of 4.5% was discovered at the 6-west bleeder connectors. Mine was avacuated and a ventilation change was made to reduce the methane content in the bleeder system.
Mine examiner and state inspector were making the BMP in #4 right off the #1 right of the Fields Fork Mains and found several falls in which had fell above the anchorage points in the travelway.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN #10 ENTRY RETURN OF NORTH MAINS. FALL MEASURED 45' X 20' X 3-6'.
A FALL OF THE ROOF OCCURED ON THE #1 UNIT, #6 ENTRY, X-CUT #6 AT THE 3RD NW PANEL. THE FALL MEASURED 85'X 19'X 5'. NO INJURIES, VENTILATION WAS RESTORED AFTER THE FALL. A ROOF BOLTING MACHINE WAS BLOCKED BY THE FALL.

Notified that vacant abandoned shaft on fire, there was not any electrical power supplied to building, the fire burned app 1 hour, Hazletownship fire company extinguished the fire. There were no injuries. There was property damage only with no value to the property.

Due to excessive rainfall this morning, June 15, 2004, the quarry is not running. We do not know at this time when normal operation is expected to resume.
Fall is 8' wide, 65' long, 5' thick, bolted with 4' resin and roof straps. Area cracked in several places on development 5-20-04. Fall near spad 174. Fall is breakered off and will not be moved.
A ROOF FALL OCCURED BETWEEN #4 & #5 ENTRIES. THE 1ST WEST AT SURVEY STATION 5+10. THE FALL MEASURED 40'LONG X 18'WIDE X 6'THICK. THIS AREA HAD BEEN BOLTED WITH 5'FULLY GROUTED BOLTS. ALL APPROACHES TO THE AREA HAVE BEEN TIMBERED OFF.
A rock fall occured between the #8 and #9 entry 8 breaks inby #4 belt. The fall measured 9'L X 19'W X 5'H.
Fall is 20' x 20' x 5' thick, bolted with 4' & 6' fully grouted resin & roof straps. Area was cracked up bad on development and upon completion of mining the area was breakered off. Developed May 10th, 04. Fall is 800' from active section near spad 152. All areas are breakered off. Fall will not be moved.
The Hart St. portal elevator malfunctioned with one of our emplyees on it. A brake release contactor failed when the elevator was 20 feet from the top landing. This caused the brakes to automatically lock on the elevator. The contactor was replaced and the elevator was returned to service at 1:30 p.m.
AT APPROX. 1:10AM, A CITY WIDE POWER OUTAGE OCCURRED WHICH RENDERED THE HOIST INOP, DUE TO A BLOWN FUSE AT THE SUB-STATION. AT THAT TIME, 3 MEN WERE WORKING IN THE PRODUCTION SHAFT. UPON ARRIVING AT 1:25AM, COMMUN. WAS RE-ESTABLISHED WITH THE MEN USING A PORTABLE GENERATOR & ALL WAS WELL. POWER CO. WAS NOTIFIED IMMED. & THE MINE MNG. AT 2:10AM POWER WAS RESTORED.
THIS WAS A ROCK FALL LOCATED 3 BRKS ABOVE SPAD 1987 + OVER IN THE #3 ENTRY IN THE INTERSECTION. THIS AREA WAS BOLTED WITH FULLY GROUTED RESIN BOLTS, PARTLY CRIBBED + TIMBERED. THIS AREA WHICH IS OUTBY IMMEDIATE WORK AREA 15,000' HAS HAD SOME ADVERSE CONDITIONS. 35'LONG, 20'WIDE, 10'THICK.
Employee was bringing low-track to bottom of slope to get supplies and found roof fall. The fall was located in the #1 East Mains (No.4 belt located in these mains) break 35 and in the #3 neutral entry, fall occurred in intersection measuring approx. 20'x30' and 3-5' thick. Neither beltline or ventilation was impaired.
AT 7:50PM AN IGNITION OF COAL DUST OCCURED WITH THE #3 KILN COAL FIRING SYSTEM. THE PRESSURE RELIEF VENT WAS RUPTURED AS IS DESIGNED TO DO. THE IGNITION SOURCE IS NOT KNOWN AT THIS TIME. A FUEL BLEND OF RECYCLED PAPER PRODUCT & COAL WAS BEING USED. NO INJURIES OR PROPERTY DAMAGE.
THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROX. 270 MINUTES DUE TO THE BOTTOM DOOR SWITCH BEING GROUNDED.
CONTRACTOR EE (THYSSEN MINING CONSTR. OF CANADA) WAS CUTTING OUT DAMAGED BOLTS AND WIRE WITH A CUTTING TORCH. THE OXYGEN AND ACETYLENE BOTTLES WERE 15-20 FEET AWAY. A SPARK FROM THE MATERIAL BEING CUT IGNITED ACETYLENE THAT WAS LEAKING FROM OR AROUND THE HIGH PRESSURE SIDE OF THE ACETYLENE REGULATOR, STARTING A FIRE THAT LASTED 45 MINS. NO INJURIES OCCURRED.

A ROOF FALL OCCURRED ALONG THE 2 MAIN NORTH #3 AT 54 CROSSCUT, #4 ENTRY INTERSECTION. THE FALL MEASURES APPROX. 18 FEET WIDE, 40 FEET LONG, AND 8 FEET HIGH. THE AREA IS SUPPORTED BY 6 FOOT FULLY-GROUTED RESIN BOLTS AND CABLE BOLTS.
IN THE BOX CANYON MAINS SECTION, ENTRY 5, CROSSCUT 54 TO 55, THE CONTINUOUS MINER OPERATOR WAS GRADING FLOOR INTO THE FACE. AS THE CUTTING HEAD APPROACHED THE FACE, A METHANE FEEDER WAS IGNITED APPARENTLY WHEN WATER PRESSURE WAS LOST TO THE CONTINUOUS MINER. THE FLAME WAS IMMEDIATELY EXTINGUISHED WITH ONE FIRE EXTINGUISHER. NO INJ. OR PROPERTY DMG. OCCURRED.
SHOTCRETE FELL ONTO THE INTERSECTION OF CONVEYORS S-2 & S-3, A PIECE FELL APPROX. 15' X 30'X 6' THICK. DAMAGE WAS EXTENSIVE TO HEADSECTION OF S-2. THIS FALL TOOK PLACE OUTSIDE OF NORMAL WORKHOURS, NO ONE WAS IN THE MINE AT THE TIME. THE FALL WAS DISCOVERED ON MONDAY MORNING. NO ONE WAS ALLOWED TO ENTER UNTIL THE AREA WAS SECURE.
Was cutting out an old come-along hook, which was inside a metal frame. He cut the hook from the frame using a 5/32"" diam. chamfered rod. Sparks from the cutting process fell onto a rag.The sparks caused the rag to smolder, he put it out by using his four pound hammer. Throwing the rag into the dumpster outside. Later noticed the dumpster on fire. Fire was 45 min in length.
A roof fall in the 1South at cross-cut #40 in #3 entry measuring 25ft x 18ft. x 6ft. - 9ft. The area was mined in April 2004 and supported with 6ft. Tension Rebar and 9ft. Mega Bolts. The fall was caused by an undetected slip.

A ROOF FALL HAS OCCURRED AT THE ABOVE MENTIONED MINE SITE AT THE LOCATION SPAD 6965, APPROX 2000 FT FROM THE WORKING FACE. NO EQUIPMENT INVOLVED, HOWEVER THE FALL MEASURED 30' IN LENGTH, 20' IN WIDTH AND 6' HEIGHT. 42"" RESIN BOLTS WERE USED IN SHALE STRATA. THERE IS NO RIDER SEAM. THE FALL WAS BREAKERED OFF AND NO REHABILITATION IS PLANNED.

A rock fall occurred when roof became saturated with water, area had been rail barred and also had been cribbed and 12 ft cable bolts installed. We were in process of removing loose top with miner in belt when it occurred.

A roof fall occurred measuring 60' long by 19' wide, beginning 10' in by spad #325. No equipment was involved, no ventilation was disturbed, no injury occurred and the fall will not be cleaned up.
During a shift change, the hoist operator noticed that the hydraulic oil had a milky white appearance to it. The hoist was shut down until the hydaulic oil in the hoist brake system was drained, flushed and replaced. No employees were in the mine.

At 8:00am a roof fall was found at #1 East Main, # 5 entry one crosscut inby. Distance from the active face is approximately 4000 feet in the intake. Size of fall is approximately 50 feet long by 20 feet wide and 5 feet thick. The fall was breakered off. These works were developed in 1994.
At 8:20 am #2 hoist, west motor cooling fan motor bearings failed, and hoist had to be shut down. back up motor will be installed today. New motor is being re-built.

The fall occured in the#7 entry, while taking the first cut out of a right break the roof fell. The fall started at the head of the miner and fell through the intersection. The fall was approx. 30ft by 40ft by 5ft thick. The roof was a busted up shale.

Non injury roof fall was discovered on the ""17 side"" of mine at the 3rd set of seals while foreman was conducting his pre-shift inspection. The fall crushed out the #1 seal and had to be replaced. This fall occurred sometime between the end of the shift on 6-5-04 and the pre-shift inspection on 6-7-04.

AN UNPLANNED WATER INUNDATION WAS ENCOUNTERED AT THE #2 TRACK ENTRY AT X-CUT 17 BY THE #3 SECTION MIDNIGHT CREW. THE WATER WAS APPROX. 36"" DEEP IN #2, 3 & 4 ENTRIES.
A NON-INJURY ROOF FALL OCCURED IN THE WEST MAINS RETURN IN THE #7 ENTRY X-CUT 29. THE FALL MEASURED 25'LONG X 14'WIDE X 8-12'HIGH. ADDITIONAL ROOF SUPPORT HAD BEEN INSTALLED ON THE LOW SIDE OF ENTRY AND IN THE X-CUT 1-2 ENTRY.
THE ELEVATOR AT EAST FINLEY PORTAL WAS DOWN FOR MORE THAN 30 MINUTES BECAUSE OF A SHORT IN THE SCR BOARD. THERE WERE 10 PERSONS ON THE ELEVATOR AT THE TIME. THERE WERE NO INJURIES AND NO DAMAGE TO EQUIPMENT.

Mantrip caught on fire and burned for about 40 minutes. Mantrip sustained damage to engine / drive train components. Cause of fire is not known.

A roof fall occurred in entry #4 main south at survey station 33 and 20. The fall measured 120' long by 19' wide by 8' thick. This area had been supported with 5' fully grouted resin bolts. The area will be cleaned up and resupported.
A roof fall was found during an exam in the return entry in an inactive are of the mine. Fall measured 43' L 19' W and up to 7' thick. Fall occurred due to wet laminated shale in the immedate roof. Roof supports in use was 5' torque tension bolts. Fall did not impeade travel or ventilation, no accidents resulted from the fall. Information was recived on the fall 06/21/04.

A ROOF FALL OCCURRED IN THE 4TH SOUTH PANEL OFF OF THE MAIN WEST AT #28 CROSSCUT, #2 ENTRY. THE FALL MEASURED 50 FEET LONG, 18 FEET WIDE & 6-12 FEET THICK.
A roof fall occurred on the 2L4W-2L panel, entry 1-0, cross-cut 13. The cause of the fall was weak laminated sandy shale. The fall included laminated sandstone streaks.
INUNDATION OCCURRED AT THE 1ST SOUTH PANEL SEALS WHICH ARE OFF THE MAIN EAST INTAKE. FOREMAN FOUND DURING DAILY INSPECTION OF THE MINE. HE WALKED OVER TO CHECK THE #4 SEAL WHEN HE SAW WATER COMING OUT OF THE 4"" RISER. SOON AFTER HE FOUND WATER COMING OUT OF THE 4"" U-TUBE IN THE #1 SEAL. MINE EVACUATED. MSHA 103-K ORDER ISSUED; LIFTED 6/8/04.
A FALL WAS DISCOVERED IN THE #3 ENTRY OF 1 LEFT OFF 1 RIGHT SUB-MAINS OFF 2 EAST (1 BREAK OUTBY SURVEY STATION #905). THE FALL APPEARS TO BE A CONTINUATION OF A PILLAR FALL AND IS ESTIMATED TO BE 90' LONG X 19' WIDE X 12' HIGH. THERE WERE NO INJURIES NOR EQUIPMENT INVOLVED. PASSAGE OF PERSONS WAS NOT AFFECTED AND VENTILATION WAS NOT AFFECTED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #3 ENTRY AT #11 X-CUT OF THE PANEL BEING PILLARED. FALL OCCURRED IN THE INTERSECTION AND HEAD OF CONTINUOUS MINER WAS CAUGHT FOR SHORT PERIOD OF TIME. NO INJURIES.
A LARGE VOLUME OF HOT CLINKER WAS DEPOSITED ONTO THE HIGH TEMPERATURE CONVEYOR BELT IN THE CLINKER STORAGE BUILDING. THE HOT CLINKER IGNITED THE BELTING MATERIAL, CAUSING A FIRE. MOUNTAIN CEMENT COMPANY MANAGEMENT MADE THE DECISION TO LET THE FIRE BURN AND NOT EXPOSE PERSONNEL TO THE FIRE OR FUMES. FIRE LASTED APPROX. 50 MINUTES. PROPERTY DAMAGE ONLY.
A fall of roof occurred in the 6th S.E. panel x-cut 27 #5 unit in #7 entry. The fall measured 46' long by 20' wide by 6' high. No equipment, ventilation, or personnel were affected.
A ROOF FALL OCCURED IN THE #4 ENTRY MAIN SOUTH @ SURVEY STATION 3 + 60. THE FALL MEASURED 40' LONG X 19' WIDE X 15' THICK. THIS AREA HAD BEEN BOLTED WITH 5' FULLY GROUTED AND 9' DOUBLE LOCK BOLTS. THE AREA WILL BE CLEANED UP AND REBOLTED.
AN UNPLANNED ROOF FALL OCCURRED IN THE #5 ENTRY INTAKE ENTRY NEAR SPAD #1229 IN TRACK ENTRY. THE FALL MEASURED 70' LONG, 20' WIDE, 5 TO 7' HIGH. ALL ENTRANCES HAVE BEEN DANGERED OFF. NO ACCIDENTS REPORTED.
A FALL OF GROUND WAS DISCOVERED IN THE 4 - 5480 N LEVEL OF THE MINE THAT WAS ABOVE THE ANCHORAGE ZONE OF 6' SPLIT SET BOLTS. THE AREA AFFECTED WAS APPROXIMATELY 9' X 9' X 20'.

A GUIDE WHEEL ON THE ELEVATOR WAS FOUND TO BE DAMAGED AND ELEVATOR WAS DOWN FOR MOST OF THE SHIFT.

A roof fall was found on 6/1/04 at 2:00am. The fall is located one break inby the #6 drive in the left hand crosscut. The fall did not involve any person or equipment. It is approx. 70ft. long x 20ft wide x 4ft-6ft thick. The fall is in an area of shale and slick rock.

A ""HORSE BACK"" (ROOF FAULT) MEASURING 2' X 14' X 38' DISLODGED FROM THE MINE ROOF PULLING 2 ROOF BOLTS FROM THE LAST FULL ROW OF BOLTS, FALLING ON THE VICTIM WHOSE LOCATION WAS NEAR THE 2ND ROW OF BOLTS, CAUSING CRUSHING BLUNT FORCE TRAUMA, RESULTING IN FATAL INJURIES BEING SUSTAINED.
AS THE VICTIM WAS WASHING DOWN THE AREA OF THE BUNKER, HE BECAME PINNED BETWEEN AN UPRIGHT BEAM AND THE RAIL BUNKER CAR. THE RESULT WAS FATAL CRUSHING INJURIES.
A FATAL FALL OF ROOF ACCIDENT OCCURRED ON THE 003 WORKING SECTION. THE VICTIM WAS WORKING WITH THE SECTION CREW AND HAD BEEN INSTALLING TIMBERS DURING RETREAT MINING. THE CREW HAD TEMPORARILY HALTED PRODUCTION TO OBSERVE THE MINE ROOF, WHEN WITH LITTLE WARNING THE MINE ROOF BEGAN TO COLLAPSE. THE 5 MINERS RAN TO ESCAPE THE ROOF FALL. FOUR OF THE MINERS ESCAPED.
WHILE HAULING DIRT DOWN THE IMPOUNDMENT HAUL ROAD, IT APPEARS EMPLOYEE LOST CONTROL OF THE 10 WHEELER TRUCK HE WAS DRIVING. HE JUMPED FROM THE TRUCK AND IT STRUCK HIM CAUSING FATAL INJURIES.
EE WAS LOWERING AN OBJECT TO THE GROUND WITH HIS BOOM AND WHEN SLACK WAS IN THE CABLES EE GRABBED THE CABLE AND RE-POSITIONED THE BLOCK AND THE OBJECT SLIPPED OFF AN UNSEEN OBJECT ON THE GROUND CAUSING HIS FINGER TO BE PULLED INTO THE PULLEY.
WHILE 2 EMPLOYEES WERE REPLACING V BELTS ON SHAKER SCREEN, PULLEY SLIPPED & CAUGHT 1 EMPLOYEE'S HAND BETWEEN V BELTS AND SHIVE. THE EMPLOYEE'S LEFT HAND WAS HURT. HIS 4TH (RING FINGER) WAS SEVERELY MASHED AND IT CUT THE END OF HIS 5TH(PINKY FINGER) OFF AT THE FIRST JOINT.
TWO EMPLOYEE'S WERE LEAVING NO 2 ENTRY TO GO TO THE TRACK TO GET TIMBERS. INJURED RESTED HIS RIGHT HAND ON THE RAM TO REMOVE A SAW FROM THE SCOOP BUCKET. CO-WORKER PULLED THE RAM JACK LEVER TO RELEASE THE BRAKE. THE RAM SMASHED HIS RIGHT HAND. HIS INJURY CONSISTED OF AMPUTATION OF THE RIGHT MIDDLE FINGER AT THE FIRST JOINT.
Ee was turning left hand nich with miner. He was on his knees on right side of miner when a rock fell out between bolt and rib hitting him on shoulder then falling on his right foot cutting off his 3 little toes.
EMPLOYEE WAS CUTTING AN OLD TOOL BOX FROM BACK OF HIS VEHICLE WHEN THE LID CAME DOWN AND SEVERED HIS RT.HAND, LITTLE FINGER FROM THE FIRST JOINT.
Not following established procedures and improper use of safety equipment. EE attempted to unhitch an air compressor trailer from pickup when the trailer dropped on his R. foot, severing his R. toe and smashing 2 others. EE was assessed by the attending Physician Asst. at mine clinic. EE did not use trailer stand. EE lost his balance in the sand, dropping tongue on R. foot.
WHILE TIGHTENTING BOLTS ON SCREEN EMPLOYEE PUT HIS HAND ON V.BELTS. BELTS MOVED PINCHING PINKEY FINGER BETWEEN BELTS & SHIVE.
EE had put up his outside bolt and began drilling the hole for the inside bolt when the place fell in, the atrs broke the rock but a piece fell back under the atrs striking him on the right leg, fracturing his leg. the cut was approx. 7 1/2 feet high. the piece of rock that struck him was approx. 40""w x 60"" l x 4"" thick.
EMPLOYEE WAS REPAIRING A DAMAGED RUBBER AIR HOSE. HE WAS TIGHTENING A BREEZE BAND WITH A SCREW DRIVER. THE SCREW DRIVER SLIPPED OUT OF SLOT IN BREEZE BAND AND PUNCTURED LEFT HAND BETWEEN THUMB AND FIRST FINGER.
EE WAS USING PRY BAR TO REMOVE ROCK FROM MINER. STRAINED BACK.
EMPLOYEE WAS CUTTING A TREE WHEN AN UNSEEN GRAPEVINE IN THE TOP OF THE TREE PULLED A DEAD LIMB (6' BY 8"") ON HIM, BRUISING HIS SHOULDER AND RIBS.
The employee was cutting a steel plate when the cut end fell on his left foot. He told the head mechanic he told the owner, who then told injured to go to the doctor as the steel had punctured the skin. injured filled a L&I report. On the report, injured said that head mechanic was a witness, but he did not witness the accident.
ROCK FELL OFF PRODUCTION TABLE ONTO THE BACK OF HIS LT. FOOT (HEEL AREA); IT BROKE A COUPLE BONES IN HIS FOOT.
Employee sustained a fracture to his left tibial plateau behind knee when he jumped from a moving motor.
EMPLOYEE WAS OPERATING A SCOOP WHEN A PIECE OF DRAW ROCK FELL & HIT HIM ON THE RT. SHOULDER BLADE & RT. SIDE OF HIS BACK.
Contused and sprain left knee when it was caught between a motor base and belt structure.
EE was working in GBF modules. The modules were full of raw kiln feed. He was using a vac hose and grinding screens. His skin was moist from sweating. It came in contact with akaline dust, resulting in alkali burns to both shins, both forearms and both hands. The burns to his shins required skin grafts.
Employee was going to start up the Iron Pyrite circuit and went to turn on the safety switch box lever. The lever got stuck half way up and he lower it, he then tried to lift the lever up again with some force and the electricity arced burning his left hand.
Forklift was being driven, unloaded, back to the loadout area. The pavement had been wetted to suppress dust. Employee was traveling too fast, swerved to miss a pallet in the line of travel. Forklift spun 120 degrees. His right rear wheel slid into ditch and the left rear side of the forklift climbed a berm along side of ditch. Forklift overturned on its right side.
EMPLOYEE WAS DUMPING FLY ASH AND THE TRUCK SLOWLY UPSET TO THE LEFT SIDE.
Employee was lifting bags of pigment and dumping into mixing pot when he felt a pull in his left back muscle
Pulling steel rods out of hole - when the drill head was screwing to the rod to tighten he put his hand on the metal stop that the wrench hits against and it smashed his finger between the wrench and the metal shop.
Ee had unloaded some brake calipers out of the bucket of the 910 loader. His back started hurting later.
Got out of tractor/truck to clean windshield. Lifted hood on tractor. Climbed up on diver side tire to clean the windshield, foot slipped. Fell between the hood & bumper, stuck left rib cage.
Employee was working on the back side of unit belt tail. The belt went off for unknown reason. as he was crawling across top of belt it started up carrying him down the belt for approx. 70'. The area is very low and he hit the mine roof several times.
EMPLOYEE WAS ASSISTING WITH CRUSHER MAINTENANCE WHEN THE WIND PICKED UP AND BLEW SOMETHING INTO HIS LEFT EYE. THE EMPLOYEE WAS WEARING HIS SAFETY GLASSES AT THE TIME.

Injured party lifted the miner cable on #1 unit to hang, without waiting for help and felt pain in his lower back, Injured party continued to work and did not go to the doctor or miss any work until 7-2-04.
EMPLOYEE WAS CUTTING TREES ON A STEEP SLOPE. WHEN THE TREE BEGAN TO FALL A LIMB BROKE OUT OF THE TOP OF THE TREE. EMPLOYEE RAN TO ESCAPE THE FALLING LIMB CAUSING HIMSELF TO FALL DOWNHILL APPROXIMATELY 20 FT.
USING A CUTTING TORCH IN GARAGE NEAR A BUCKET OF UNKNOWN FLAMMABLE MATERIAL. SPARKS FROM TORCH IGNITED IT, RESULTING IN AN EXPLOSION AND FIRE.

Employee received burns when a flash occurred while installing shaft sleeve on gear reducer.
EE was cleaning under a slag feeder with a skid loader. There was 3 inches of clearance between the top of the loader and the bottom framework of the feeder. He went under the feeder and filled the loader bucket. When he reveresed the loader to come out, it lurched upwards pinching the 3rd and 4th fingers of his left hand between the feeder and loader controls.
EMPLOYEE WAS CLIMBING OUT OF RAIL CAR WHEN HE SLIPPED AND FELL TO THE GROUND BREAKING HIS LOWER LEFT LEG.
Employee said he raised lid on battery to water the batteries on the 488 scoop and that the lid fell, striking him on the hand, left hand, hurting middle index finger.

Was loading wet bags, went to pick one off the ground & back went out.
PULLED MUSCLE IN LOWER STOMACH AREA. WAS PULLING ROCK BY HAND. WASN'T USING AUTOMATIC ROLLERS THAT ARE PROVIDED.
While unloading posts from the supply car to a ram car ee was trying to reposition the posts. @ posts rolled down pinning his right forearm between the post and car.
Ee states, he was pulling on rope hooked to miner cable and slipped and fell hurting back.
Injured had completed setting survey station (#555) in the face area when a piece of draw rock fell and hit him on his right shoulder.
EMPLOYEE REPORTED: WHEN BENDING AT THE WAIST TO PICK UP A CONTINUOUS MINER CABLE, PAIN WAS FELT IN HIS LOW BACK. HE NEVER CONTACTED THE CABLE, THE PAIN WAS FELT WHEN BENDING.
EE was pushing in the shot with a d11, a rock rolled in behind him and when he backed up he hit the rock, causing a hard jolt. He felt something pop in his neck.
5NW SECTION OUTBY FEEDER IN #6 ENTRY. EMPLOYEE GETTING OUT OF SCOOP (OPERATORS COMPARTMENT), NO CANOPY, HE STOOD UP & HIS FOOT SLIPPED ON MUD & OIL ON THE DECK & HE FELL BACKWARDS. HIS BACK HIT CORNER OF SCOOP AS HE FELL OUT OF THE DECK. POSSIBLE LOWER BACK INJURIES.
BELLY DUMP GATES DIDN'T OPEN. TOLD DRIVER TO GO TO YARD. HE DID NOT GO TO YARD BUT TRIED TO OPEN GATES HIMSELF BY BUMPING THEM. DRIVER PUT TRUCK ON UNLEVEL GROUND, TIPPED TRUCK ON IT'S SIDE. POOR ACTION ON PART OF DRIVER.
Injured was helping to install arches when piece of rock fell from rib and struck back of hard hat causing a contusion.
Employee stepped over track to reach jacket, twisted his right ankle.
EE bent over to pick up a concrete block for the brattice he was building. He felt pain in his lower back. He continued to work up until 10-18-04. He had surgery to repair a disc in his back on 10-19-04.
DID NOT START LOSING TIME UNTIL 8/25/04. Twisted left knee when he tripped on track switch while walking back to locomotive.
Ee was loading railcars. As he was getting off the railcar, his foot landed on a rock & twisted his ankle/foot.
Employee was traveling down the face following the shear, a shield failed to move in on auto and he reset the controls moving the shield in. As he turned to walk away a piece of coal struck him on the cleft cheek and air stream helmet causing lacerations and a fractured cheek bone.
EMPLOYEE HAD FINISHED FUELING AND GREASING DOZER. HE WAS COMING BACK DOWN OFF TRACKS AND SLIPPED AND FELL OFF DOZER.
EMPLOYEE, SITTING ON A SHUTTLE CAR, TOOK HIS FEET AND PUSHED THE MINER CABLE OUT OF THE WAY. HE SPRAINED HIS KNEE PUSHING ON THE MINER CABLE.
Ee was riding mantrip when mantrip struck truss bolts stacked by tracks. Truss bolts struck ee on right arm.
Employee was descending the ladder from his haul truck when he stepped onto the ground he stepped on a rock about the size of a salt shaker. The rock rolled causing the employee to twist his ankle and fall. As he fell he tried to catch himself with his left hand, causing a sprained wrist and elbow contusion.
EMPLOYEE STEPPED DOWN FROM COMPACTOR TURNING HIS LEFT LEG CAUSING MUSCLE STRAIN. WET CONDITIONS WERE A CONTRIBUTORY FACTOR.
EMPLOYEE ALLEGES HEAD INJURY ON 6/26/04. HOWEVER, EMPLOYEE DID NOT REPORT FOR WORK. WORKMAN'S COMPENSATION CLAIM IS BEING CONTROVERTED.
While helping throw a piece of pipe, ee fell and hurt his right hand.
EMPLOYEE EXPERIENCED PAIN IN LOWER ABDOMEN WHILE LOADING A BOX OF RESIN ONTO A ROOF BOLTER. EMPLOYEE WENT TO DOCTOR AND WAS DIAGNOSED WITH A HERNIA.
Employee stated he was washing out bed of ram car to allow bed to retract. Employee stated he slipped while handling ram car and injured ribs.
EMPLOYEE WAS WALKING ALONG THE TRAVELWAY WHEN HE HEARD A NOISE. AS HE LOOKED UP, A ROCK FELL, STRIKING HIM ON THE HARD HAT LIGHT & BILL, PUSHING HIS HEAD & NECK DOWN & BACK.
Ee stated that he was outby the last open x-cut when a piece of draw rock approx 2 feet x 2 feet x 1/8 inch thick struck him on the back. He continued to work until 6-28-04 when he consulted a Dr.
Employee was positioning b1 discharge nackboard when his right foot got caught between backboard and structure.
Grates used for guarding were stacked against a wall. He was trying to lift one that fell on the ground. It was partially under the others. As he lifted, the other started to fall forward. He caught them with his hands and thigh. They came to rest on his thighs and knee cap. No cuts, there was bruising and soreness to knee.
EMPLOYEE STATED THAT HE HAD TAKEN THE DRILL STEEL OUT OF THE DRILL POT & STOOD IT ON THE ROOF BOLTER PLATFORM. AFTER INSTALLING THE ROOF BOLT HE WAS REMOVING THE WRENCH AS HE WAS LOWERING THE DRILL POT & AT THE SAME TIME THE ROOF BOLTER MAST CAUGHT THE DRILL STEEL & BENT IT LIKE A BOW PINCHING HIS RT.INDEX FINGER ON TOP OF THE DRILL WRENCH. FRACTURED & LACERATED RT. INDEX FINGER.

THE ACCIDENT HAPPENED IN MAIN A#S. THE BOLTER WAS BOLTING THE BELTLINE, THE OPERATOR OF THE ROOF BOLTER WAS OBSERVING THE ROOF WHEN THE RIB BROKE FREE FROM THE CORNER HITTING THE OPERATOR IN THE FOOT. THE OPERATOR INJURED HIS LEFT FOOT.
Employee was running roof bolter and rock fell from top striking him on the back of right hand. Started missing work 6/28/04.
Employee was removing track component cap on a D11 Cat crawler when the support failed, pinching his finger between the tool he was using and the dozer track.

EMPLOYEE WAS LOOSENING A SCREW WHILE ADJUSTING THE TENSION ON THE JAW(OF THE CRUSHER). THIS SCREW SEPARATED FROM THE PIECE WHICH HOLDS THE SPRING IN PLACE HITTING HIM & CAUSED A LACERATION WITH CONTUSION ON THE RT.THIGH/LEG.
EMPLOYEE WAS STEPPING FROM HAUL TRUCK TO CATWALK AT SECONDARY PLANT WHEN HIS FOOT SLIPPED AND HE FELL ONTO HIS BACK.
Employee was over valve, starting to open the 10' valve when victalic coupling blew apart due to the mainline being under presser 200 psi the force of the uncoupled pipe hit employee on right shoulder and ribs, knocking him to the ground.
EE WAS PULLING ON WATER LINE IN MUDDY AREA. SLIPPED AND FELT PAIN IN LOWER BACK.
Employee was getting out of mantrip when he felt his knee pop.
Employee developed a pain in the lower back after operating a Caterpillar D8 bulldozer.
EMPLOYEE WAS A PASSENGER IN #23 MAN BUS THAT HAD STOPPED AT THE HAND SLAP FOR #3 DERAIL WHEN IT WAS STRUCK IN THE REAR BY #27 MAN BUS. ON IMPACT THE EMPLOYEE STATED THAT HE HIT HIS HEAD ON THE BULK HEAD OF THE PASSENGERS COMPARTMENT AND THEN WAS THROWN FORWARD INTO THE DECK OF THE COMPARTMENT.
EE WAS STACKING STONES ONTO A PALLET AND HE PINCHED HIS FINGER BETWEEN TWO STONES CAUSING HIS LEFT RING FINGER TO FRACTURE.
EE WAS WALKING UP THE HILL ON THE #1 MAINS BELTING AT BREAK #18 WHEN HE SLIPPED & FELL. EE CONTINUED TO WORK UNTIL THE PAIN IN HIS LOWER BACK WORSENED. HE SAW SEVERAL SPECIALISTS & WAS DIAGNOSED WITH A HERNIATED L4 & L5. RECOVERY TIME IS UNKNOWN. NOTE: HIS FIRST DAY MISSED WAS 8-10-04.
Employee was fabricating a reinforcing mat of rebar for concrete. He was burning a piece to size when a spark went under his safety glasses and went into his eye in the pupil area, becoming lodged.

EMPLOYEE WAS IN THE PROCESS OF INSTALLING A ROOFBOLT. WHEN HE SQUATTED DOWN TO RETRIEVE A ROOFBOLT HE FELT HIS KNEE POP.
EMPLOYEE WAS USING A COME-ALONG ON A D 11 DOZER WHEN HIS HAND SLIPPED CAUSING EMPLOYEE TO FALL OFF ONTO THE TRACKS OF THE DOZER.
The Employee was holding the track for the crane operator, when the operator backed up too fast causing the track to swing. The track hit the Employee in the left foot knocking him to the ground
EMPLOYEE WAS REMOVING THE HAMMER PLATES FROM THE SECONDARY CRUSHER, WHILE HAMMERING WITH A SLEDGE HAMMER TO DETACH THE PLATES, A METAL PARTICLE OF APPROX. 1mm SNAPPED OFF THE PLATE, IMPACTING EE'S RIGHT LEG. THE METAL FRAGMENT CUT INTO THE SKIN & BECAME EMBEDDED.
The injured was dragging with S&S tractor and drag. He ran over a piece of rock in the road, this bounced the tractor and threw him up against the canopy on the tractor, straining his neck muscles.
EE STATED HE WAS WALKING FROM CEDAR CREEK ELEVATOR TO GET A LOCOMOTIVE (APPROX. 2 X-CUT) AND SLIPPED WHILE WALKING AND INJURED HIS RIGHT ANKLE. EE'S FIRST LOST WORK DAY OCCURRED ON 6/28/04.
EMPLOYEE WAS TRYING TO STRAIGHTEN A 4""X 21'PIECE OF PIPE. HE WAS KNEELING ON THE GROUND & TOLD THE LOADER OPERATOR TO MOVE FORWARD, WHEN THE LOADER MOVED THE PIPE FELL OFF THE BLOCKS & STRUCK HIS KNEE.
WHILE SHIVING A ROLLER, WHEEL HIT HIM IN THE CHEST KNOCKING HIM DOWN. UPON HITTING THE GROUND, HIS HEAD HIT A COBRA DRILL.
He was helping put cribs blocks under the bumper of the miner, he left his right hand on a crib block as the operator lowered his stab jack, causing his two fingers to be smashed.
INVESTIGATION AT THIS TIME INDICATES THAT WHEN EE WENT TO LIFT A MAULER PADDLE, HE FELT A STRAIN IN HIS BACK, BUT DID NOT IMMEDIATELY REPORT IT. WE DID NOT FIND OUT ABOUT IT UNTIL 7/06/04 - IT IS STILL UNDER INVESTIGATION.
Employee was stepping down off a shield car handling a belt structure and his ankle popped. Cause of this injury was concentration. Compliance with rules/reg was not a factor, protective equipment was not a factor, miner proficiency a factor, mining equipment/system not a factor.
EE was throwing roof bolts into tray and his finger was smashed between bolt and tray.
EE WAS MOVING A ROCK FROM BENEATH THE TIRE OF A TRAILER BEING UNLOADED. EE PULLED ON THE ROCK, ROCK MOVED, EE'S HAND SLIPPED ALLOWING THE ROCK TO ROLL BACK INTO ORIGINAL POSITION, PINCHING EE'S RIGHT MIDDLE FINGER AGAINST ANOTHER ROCK. THE LACERATION REQUIRED SUTURES.
While traveling up a ramp on a crane, ee lost traction and coulnt regain it. Crane was backed 20' into the berm & left rear wheel rolled up on the berm tipping the crane over and threw ee thru the window open top cab roof that was in an open position. crane is a front wheel drive with non rops cab and no seat belt
EE WAS WALKING BY THE PANEL OF THE FEEDER AND STATED THAT HE SLIPPED/TRIPPED AND CAUGHT HIMSELF WITH HIS LEFT HAND IN THE PANEL, RESULTING IN AN ELECTRICAL BURN. EE WAS TRANSPORTED TO CAMC GENERAL AND TO WESTERN PENNSYLVANIA HOSPITAL WITH 3RD DEGREE BURNS. THERE WERE NO WITNESSES TO THE ACCIDENT.
He was pulling supply cars into 8 north when he thought the trip was pulling hard. he looked back to see if a car had come off track & when he did,he hit the back of his head on 1A overcast.he continued to work the remainder of his shift&and said that his neck was stiff and was experiencing a headache. his hardhat hit overcast, not his head. first day missed was 7/12/04.
EE was sitting on bench at bottom of shaft. When he got up to leave he bumped head on door. Has had stiffness in neck since. EE was wearing a hard hat.
Ee was in the process of dumping a load of rock when he backed over a 2 foot trench. He complained of a lower back injury, his diagnosis is pending.

EE HAD GONE TO BACK OF BOLTER TO GET A BOX OF RESIN GLUE. WHILE LIFTING THE BOX HE FELT A BURNING SENSATION IN HIS LOWER BACK.
EMPLOYEE WAS REMOVING STONES FROM THE CONVEYOR & PLACING THEM ON A PALLET WHEN HE BEGAN EXPERIENCING PAIN IN HIS BACK. DIAGNOSIS: LUMBAR STRAIN.
LIFTING TOOLBOX ONTO PICKUP BED, HE STRAINED HIS BACK.
Employee was welding on a loader bucket. A chain hoist was attached to the bucket. The nylon strap broke and the chain host swung down and struck employee's left hip. His hip was bruised. Injured is losing time.

EMPLOYEE WAS WALKING TO THE ROOFBOLTER WHEN ROCK FELL OUT FROM BETWEEN THE BOLTS AND STRUCK EMPLOYEE ON THE SHOULDER AND BACK.
The serviceman (greaser) was helping a welder install a handrail on a dozer when he grabbed an unsecured rail causing him to lose his balance and fell backwards. He landed on concrete floor with both feet.
Pinched left hand between driveshaft and loader frame while removing driveshaft.
Employee felt a ""pull"" in his groin area while moving supplies on his truck bed. He continued working and was later diagnosed with a hernia.
Approx. 10 am a power pole near the slurry wash out station arced and popped. Someone noticed sparks from the pole. At 3:00 pm our employee complained to our plant manager that he was not feeling well from the odor that seemed to be around the wash station. He said he had even gotten sick and thrown up.
EE STATED THAT WHILE TIGHTENING THE BELTS A CHAIN BROKE AND STRUCK HIM IN THE ARM.

THE EMPLOYEE WAS WALKING UNDER THE CONVEYOR WHEN A 3 TO 4 INCH STONE APPROX FELL ON HIS HEAD. THE EMPLOYEE HAD SECURITY HELMET BUT DIE TO THE IMPACT AND SIZE OF BOULDER EMPLOYEE SUFFERED A CUT TO HIS HEAD AND DIZZYNESS.
FROM STANDING ON A SLOPED INCLINE FOR AN EXTENDED PERIOD OF TIME WHILE WELDING ON A NOSE ROLLER, THE EMPLOYEE'S HIP BECAME SORE. THE INCIDENT HAPPENED ON 6/22/04. HE TRIED TO WORK THROUGH IT. HIS FIRST MEDICAL TREATMENT WAS ON 7-9-04. ON 7-12-04 HE WENT BACK AND WAS TOLD TO TAKE SOME TIME OFF WORK.
Employee was tramming the roof bolter from the deck and had his foot hanging out of the bolter when he got his foot caught between the roof bolter and the rib in the number 5 entry.
A SCOOP CAME THROUGH A WING CURTAIN IN THE LAST OPEN CROSSCUT, WATERING ROADWAYS. THE SCOOP BUCKET STRUCK THE BACK REST PLATE OF THE COAL HAULER BEING DRIVEN BY EE. THE IMPACT CAUSED A WHIPLASH EFFECT; THE MUSCLES IN EE'S NECK STARTED CRAMPING AFTER THE SUDDEN IMPACT TO THE SIDE & BACK OF THE COAL HAULER CAB. SCOOP DRIVER DID NOT SEE THE COAL HAULER UNTIL THE IMPACT.
Installing hyd. oil into tank off dozer, while exiting machine, slipped off of the push arm, onto the ground twisting his left ankle.
Injured was helping install an 8' timber - when the timber touched the roof - a piece of rock measuring 2"" x 5"" x 15"" fell striking him on his miner's hat.
EMPLOYEE WAS IN PROCESS OF CHANGING HOSE ON LONGWALL SHIELD & WHILE PULLING ON HOSE IT SUDDENLY CAME FREE CAUSING HIS LEFT ELBOW TO STRIKE BASE OF SHIELD HYDRAULIC JACK, BRUISING ELBOW.
EMPLOYEE WAS OPERATING AN OPEN DIESEL PERSONNEL CARRIER. HE WAS TRAVELING ALONG THE UNIT 1 TRAVEL WAY. THE BELT CREW WAS PULLING THE UNIT 1 BELT TOGETHER. THE BELT WAS BEING PULLED W/A DIESEL SCOOP ATTACHED BY A WIRE ROPE. THE DIESEL SCOOP PULLED ACROSS THE TRAVEL WAY AND EMPLOYEE RAN INTO THE CABLE. THE INCIDENT CAUSED A LACERATION TO HIS FOREHEAD REQUIRING STITCHES.
When coupling 4 inch pvc pipe together, by stricking one end of the pipe with 2 foot 2x4. when swing 2x4 he felt a pain in his right elbow.
Conveyor was being held off ground by chain hooked to excavator while conveyor axle was being removed. Load shifted, conveyor rolled back, chain came undone and conveyor fell to ground hitting and pinning employee.
EMPLOYEE STATES HE WAS PLACING A CRIB BLOCK UNDER THE PANLINE. THE PANLINE FELL, CATCHING HIS HAND BETWEEN PANLINE AND CRIB.
Employee was mucking out a round with a loader. The bucket became lodged on a rock or an undercut on the face. The employee rolled the bucket back causing the rear end to raise up. The bucket suddenly slipped off the face and the release of mechanical energy caused the loader to slam down hard on the ground sending a shock through the machine to the operator
EMPLOYEE WAS OPERATING SCOOP, CLEANING OUT BY SECTION. CANOPY CLIPPED ROOF STRAP, FORCING STRAP UNDER CANOPY, STRIKING EMPLOYEE IN RIGHT SIDE OF FACE AND NOSE RESULTING IN A SEVERE CUT IN RIGHT JAW AND CUT UNDER RIGHT EYE. SAFETY GLASSED PREVENTED SERIOUS INJURY TO EYES.
EMPLOYEE WAS HELPING LIFT HIGH VOLTAGE CABLE ONTO TRAILER AND FELT PAIN IN MID-SECTION. SCHEDULED FOR HERNIA SURGERY
EE WAS TAMPING A FEEDER HOLE WHEN HE TWISTED AND FELT PAIN IN HIS LOWER RIGHT HIP THAT RADIATES DOWN HIS LEG AND CAUSES PAIN AND NUMBNESS IN HIS RIGHT FOOT.
Ee stated back had been hurting for about a week after rock fhit in shoulder. took to hospital having muscle spasm did not miss any work with back until 7-13-04-went back to dr with muscle spams and taking physical therapy.
EE WAS PUTTING JACK ON SCOOP AND STRAINED HIS BACK. EE DID NOT START MISSING WORK UNTIL 11/11/04.
The employee was pulling rock dust hose down beltline. He slipped and fell onto his lt. elbow. Diagnosed with torn tendon. Had surgery on 10/21/04. First day missed was 10/21/04.
EE WAS WALKING IN PIT & DECIDED TO ROLL A LUMP OF COAL OFF THE 3' HIGH EDGE OF THE MINED AREA. IT ROLLED AGAINST HIS ANKLE CAUSING SOME DISCOMFORT IMMEDIATELY. EE TRIED TO WORK BUT ENDED UP MISSING TIME DURING THE COURSE OF THE NEXT MONTH DUE TO SWELLING.
Truck driver smashed his left index finger in the tailgate latch while attempting to close the tailgate. Driver sustained a fracture of the left index finger and an open wound.
EE WAS USING TIE WIRE TO HANG VENT TUBE. AS EE PULLED DOWN ON ONE END THE OTHER END OF THE WIRE CAME UP AND WENT BEHIND HIS SAFETY GLASSES AND PUNCTURED HIS LEFT EYE. LOST DAYS BEGAN 6/23/04.
Trimming belt splice knife slipped and cut left palm.
EE WAS INSTALLING 8-FT-LONG CABLE BOLTS IN #8 ENTRY INTERSECTION. WHILE PUSHING THE ROOF BOLT UP, IT GOT STUCK. EE TRIED TO PULL THE BOLT BACK OUT OF THE HOLE. THE BOLT CAME OUT OF THE DRILL POT ON THE ROOF BOLTER, AND THE ROOF BOLT PLATE STRUCK THE EE ON THE NOSE AND IN THE FACE.
While walking in #5 entry the ee stepped in a hole and felt a pain in his lower left back. the injury was diagnosed as a strain.
Employee was pulling dart from 8-2 rougher distributor. The dart turned catching flow in the distributor and pulling employee down and forward.
The employee was walking down the slope belt and stepped on a small lump of coal, slipped, twisted his upper right leg hip area. the employee did not return to work his next scheduled shift. strain to upper right leg, two days away from work.
FILLING BATTERIES ON SMALL THREE-WHEEL MANTRIP, HE STARTED FOR MORE WATER WHEN MANTRIP STARTED IN REVERSE. HIGH TRAM TRAPPED HIM BETWEEN TOP AND TOP OF MANTRIP AND ROLLED HIM IN TOP.

EMPLOYEE WAS HANDLING AND REMOVING CONVEYOR BELT STRUCTURE FROM THE BELTLINE SYSTEM WHEN HE GOT HIS LITTLE FINGER TIP PINCHED BETWEEN 2 BELT STANDS, RESULTING IN A LACERATION TO HIS FINGER TIP.
STEPPED OFF OF IMPACT LADDER ON TO A ROCK & TWISTED RT. ANKLE.
EE was at the right rear of the bolter. He bent over to move the cable, and was out of sight of the operator. As the machine moved it hit him, causing him to fall. The bolter got onto his left foot and ankle. EE suffered soft tissue damage to the left ankle.
NO. 1 SHUTTLE CAR WAS GOING OUT BY THE FACE WHERE COAL WAS BEING MINED. NO. 2 SHUTTLE CAR WAS COMING THROUGH THE INTERSECTION AND THEY HIT. THE NO. 2 OPERATOR DIDN'T SEE #1 SHUTTLE CAR OPERATOR AND HIT.
EMPLOYEE WAS INSTALLING A ROOF BOLT AND AS HE PUSHED TO STRAIGHTEN THE BOLT HE FELT SOMETHING ""POP"" IN HIS RIGHT SHOULDER. LOST TIME BEGAN 9-16-2005.

Hurt back while pulling old ball gears away from drive.
EE WAS PASSENGER IN MANTRIP WHICH LOST POWER COMING UP THE MAIN SOUTH SLOPE. DRIVER STARTED PUMPING THE FOOT BRAKE BUT IT FAILED TO HOLD THE RIDE AND THEY STARTED ROLLING BACKWARD. DRIVER PUT IT IN PARK, WHICH DID NOT HELP. DRIVER THEN SET OFF THE PARKING BRAKE BUT THEY HAD HIT THE COAL RIB ABOUT THIS TIME, INJURING EE'S RIBS.
Employee attempted to push a cut tree using one hand to fall in direction. Employee strained his back.
WHILE HANGING THE MINER CABLE, EE FELT SOMETHING ""PULL"" ALONG HIS RIGHT SHOULDER. ON 8/6/04 HE VISITED A LOCAL PHYSICIAN AND WAS REFERRED TO A NEUROLOGIST. AFTER VIEWING THE RESULTS OF HIS EXAMS, HIS PHYSICIAN REFERRED EE TO PHYSICAL THERAPY. THIS REPORT WAS DELAYED DUE TO THE LACK OF A DEFINITE DIAGNOSIS OF THE AILMENT.
MAN LEFT BUGGY IN FORWARD DIRECTION WHILE LOADING MATERIAL ON IT. PART HIT FOOT SWITCH & BUGGY MOVED FORWARD HITTING EE ON ANKLE & BROKE BONE IN IT.
Employee was attempting to change jack on roof bolter when the support broke letting the boom fall on left leg. The leg fractured and lacerated.

Employee was being loaded by a 922 Cat in a Cat D-30-D Haul Truck when large chunk shook truck, emp's strain his neck and upper back.
HE TORE HIS BICEP MUSCLE PULLING HIMSELF INTO HIS VEHICLE SEAT TO START THE SHIFT.
WHILE EE WAS WALKING BY LOAD CENTER TO RE-SET A BREAKER, HE STEPPED ON A LOOP OF CABLE AND TWISTED HIS LEFT KNEE. HE WORKED UNTIL JUY 15, 2004, WHEN HE WAS TAKEN OFF WORK.
Switching batteries on water pump, he got battery acid in his eye.
Employee was tramming a manlift from its parked location to the plant. The front wheels went into a depression of about 15-18 inches causing the basket to shake. Employee fell out of basket to the ground suffering a hairline fraction of the pelvis and a compression fracture of a vertabrae in the lower back.
EE AND ONE COWORKER WERE INSTALLING PIPE. A MONORAIL HAD BEEN CUT OFF TO ACCOMMODATE THE ROUTE THE PIPE HAD TO TAKE. EE AND COWORKER LEFT TO RETRIEVE A 1-TON CHAIN FROM THE OTHER END OF THE MONORAIL. UPON THEIR RETURN, THEY PASSED UNDER THE CUT OFF END OF THE MONORAIL, AND A SECOND CHAINFALL ATTACHED TO A DOLLY CAME OFF, STRIKING THE EE IN THE BRIDGE OF THE NOSE.

WHEN BREAKER WAS PUT BACK IN HE RECEIVED A SHOCK. BREAKER DID NOT STAY IN. INJURED EE WANTED TO WORK REST OF SHIFT BUT WAS TAKEN TO HOSPITAL TO BE EXAMINED.
EE WAS CHANGING BUSHINGS ON CRANE BOOM, MADE A SUDDEN MOVE AND FELL OFF BOOM.
Employee was standing beside the pinner. The pinner was backed up to the last open x cut. As the miner cable was pulled, the employee's foot got caught in a loop of miner cable, pulling his right leg under the pinner causing a fracture to a bone in his lower leg. He started missing work on 6-21-04.

THE HOSE, STILL ON, MOVED TOWARD THE EMPLOYEE WHO ATTEMPTED TO CATCH IT WITH HIS HAND WHEN IT SUCKED HIS HAND AND ARM INTO THE HOSE. BRUISED SHOULDER AND ARM, SPRAINED THUMB, SWELLING.

OPERATOR DAMAGED A NERVE IN HIS NECK USING A PICK AXE TO BREAK UP A PIECE OF CLAY. LOST TIME BEGAN OCT. 7, 2004 FOR SURGERY TO REPAIR NERVE DAMAGE.
THE ORIGINAL ACCIDENT OCCURED MARCH 6, 2004 AT WHICH TIME EE CONTINUED TO WORK. HE WAS EXITING THE SURFACE MANTRIP, SLIPPED & FELL TO THE GROUND, HURTING HIS BACK. HE WORKED THROUGH JUNE 18, 2004 & THEN BEGAN MISSING WORK DUE TO BACK PROBLEMS.
While bending over to pick up a piece of paper between seats in mantrip van, ee twisted his back. He has had a history of back problems. Reason being late mine has been on vacation shut down.
The employee was working on the suction hose on the roofbolter running the boom up and down and mashed his right fingers between the boom and suction hose.

WHILE ROOF BOLTING, A PIECE OF ROCK FELL FROM A CLAY VEIN AND HIT OPERATOR ON HIS BACK.
EE was removing a segment and he was pecking on it using a small sledge hammer. A piece of metal came off the segment and hit him in his lower left arm.
EE STATED THAT HE INJURED HIS LOWER BACK WHILE LIFTING THE ROOF BOLTER SHIELDS.
Employee was operating an articulating rock truck. A rock had fallen off of the truck. When employee was backing up to turn truck he ran over rock causing a jarring motion in cab. Employee experienced pain in left shoulder. Employee continued to work until 11/5/2004 when he was removed from work by his doctor.
EMPLOYEE ALLEGES THAT WHILE LOADING TWO 7200 VOLT CABLES FROM FLAT CAR, WITH THE HELP OF ANOTHER EE, THE HELPER DROPPED HIS END OF THE CABLE ONTO THE MINE FLOOR & EE DID NOT DROP HIS END. THE SECOND CABLE WAS PLUGGED IN. EE CLAIMS TO HAVE PAIN IN HIS RT. SHOULDER & NECK.
WHILE HELPING TO SET A TIMBER ON THE IN-BY CORNER OF 6XL CROSSCUT, A PIECE OF ROCK MEASURING 30"" X 40"" X 6"" FELL HITTING EE ON THE HEAD. HE WAS TRANSPORTED TO RALEIGH GENERAL HOSPITAL AND DIAGNOSED WITH A FRACTURED VERTABRE.
Employee was replacing teeth on a 330 Volvo front-end loader bucket. Kyle attempted to drive the pin out of the tooth with a mallet and struck his thumb as a result. Kyle began to loose time on 6/29/04.
EMPLOYEE WAS BOLTING ROOF WHEN DRILL STEEL BROKE. IT CAME BACK HITTING HIM IN THE RIGHT ARM, BREAKING HIS FOREARM.
Ee stated that he was using a slate bar to lift a belt rope when the bar slipped and struck him in the ribs. He continued to work until 6-22-04 when he consulted a Dr. and it was determined that he was suffering from bruised ribs.

Ee was part of a team relaying 67# plates from the floor around a corner & up stairs to a silo. Ee was at the past of the stairs passing plate from one person to another. He felt a pop on the right side of his chest after the 7th piece. It was at the end of twisting with the plate. He dropped the plate & was given 1st aid & transported to ER.

Employee was operating a CAT 980 C loader. As the machine dug into the bank of material, the seat failed (later found to be safety wired) and the employee fell backwards.
Employee was holding a Guzzler hose to remove material from the bottom of a product elevator. The material on the sides sloughed in on the hose. The employee was trying to pull back on the hose to prevent clogging when he experienced a pain in his back. Doctor indicated thoracic spasm.

THE EMPLOYEE WAS IN THE PROCESS OF CONDUCTING A SAFETY INSPECTION OF THE MILL AREA WHEN THE BACK INJURY OCCURRED. THE EMPLOYEE WAS REMOVING THE CYANIDE ANTI-DOTE KIT FROM IT'S CABINET TO INSPECT IT WHEN HE FELT PAIN IN HIS LOWER BACK. THE ANTI-DOTE KIT WEIGHS APPROXIMATELY 15 LBS.

When ee went to get off personnel carrier he experienced pain in left knee. EE continued working until July 20, 04 at which time he started missing work, now making this a lost time accident, scheduled for surgery.
Employee was walking down #3 belt line & went through low place & bumped head & stepped on loose rock & lost balance & fell twisting ankle.
Employee was operating a Genie Z-80 Manlift on the North side of the washer. While attempting to scope in the basket, a pin retainer made contact with a section of conduit and caused the manbasket boom to become jammed. Upon trying to unjam the boom, the boom suddenly thrusted upward and caused the employees head to strike a pipe.
One employee was removing clamp from spring bar on screen tower. He dropped one-half of the clamp. The clamp hit the catwalk and fell through hitting two employees on the head.
EE was walking at the Packing House during all his work shift. Few hours later due the friction between his left ankle and his safety boot caused him an ulceration.On 6-21-04 he was sent to the State Insurance fund Corp for a medical treatment
Injured and other ee were loading by hand the ATRS jack onto a mantrip. The ATRS jack was on a pallet and as they lifted the jack off of the pallet and got it about waist high injured felt a pain in his lower back.
Coal/ rock fell from roof striking hard hat, rt shoulder and right arm, employee received 8 stitches in right arm.
EE was roof bolting along the rt side of the cont miner when a piece of loose coal rib fell, pushing him against the side of the miner, resulting in a shoulder/ neck bruise/ strain.
While getting in Hammer, employee's foot slipped off the step due to mud buildup and he fell, hitting his chest against the door frame. Went to doctor who reported a cracked rib. Doctor sent employee back to work. Employee continued to work until he stated the pain was to severe; that was on 6-21-04.

EE was grinding metal plate on ""B"" screen project. He was wearing all safety equipment at the time. However, a foreign item (metal flakes) got into his right eye.

EE WAS LOADING ROCK IN THE PIT FROM THE MUCK PILE. HE GOT THE BUCKET FULL AND STARTED TO BACK UP WHEN A ROCK ROLLED DOWN THE PILE AND HIT THE LOADER BUCKET. THE IMPACT JOLTED THE LOADER CAUSING EE TO HAVE BACK PAIN.
The ee was loading roof bolt, when his right foot slipped forward and his knee went backwards, straining his right knee. His last day of work was 6-26-04.

Utilityman was helping in a routine cage change work. After the cage was changed the repairmen were tensioning the V-belts. In order to put even tension on all the belts the 18"" cage paktor sheave has to be turned manually. During this procedure the injured employee got his hand caught betweent the sheave and v-belt.

OPERATOR OF 2ND SHUTTLE CAR WAITED FOR EE TO CLEAR THE INTERSECTION. 1ST CAR HAD TO BACK UP A SHORT DISTANCE TO CLEAR THE LEFT RIB. OPERATOR OF 2ND CAR THOUGHT HE HAD CLEARED THE INTERSECTION & PULLED FORWARD STRIKING 1ST CAR.
EMPLOYEE WAS OPERATING THE FLETCHER ROOF BOLTER. WHILE BACKING UP TO GO INTO ANOTHER FACE, HE PINCHED HIS RIGHT LEG AND FOOT BETWEEN THE ROOF BOLTER AND THE JOY SHUTTLE CAR. HE WAS IN THE DECK OF THE BOLTER WITH HIS RIGHT LEG ON THE BUMPER.
EE WAS DISMANTLING PORTABLE CRUSHER, GETTING READY TO TRAVEL. LAYING DOWN FEEDER WALL. EE WENT INTO CRUSHER TO SECURE WALL. WALL FELL, PINNING BOTH ANKLES AGAINST FEEDER WALL.
Ee had just swung the drill head on the fletcher RRII roof bolter to drill hole for sister hook when his SCSR hit the swing lever thus pinching his knee to the coal rib from the drill head of the bolter.

WE RECEIVED NOTIFICATION ON 7/5/04 THAT THE EMPLOYEE HAD TORN CARTILEDGE & WOULD NEED SURGERY, MAKING THIS INCIDENT REPORTABLE.

CHANGE OF DRIVE SHEAVES WAS BEING PERFORMED ON A FEEDER BELT. THE TORK ARM WAS REMOVED FROM THE GEAR BOX & CABLE PULLER WAS USED TO ROTATE THE GEAR BOX. GEAR BOX WAS ROTATED WITHOUT SECURING THE FREE END WHICH ALLOWED THE GEAR BOX TO FALL. THIS PINNED THE MINERS HAND BETWEEN THE GEAR BOX & FRAME.
EE was using a shovel when injury happened. Injury happened on monday. EE went to dispensary on tuesday and company dr on wednesday. On 6-17-04 dr determined that ee would need surgery.
Employee cut a tree when a grapevine dislodged a rock approx 75 ft uphill from the tree. The rock rolled down the hill striking employee on the knee.
Driver turned truck to the right too quick into berm of curve causing truck to overturn.

Employee was lifting & pulling with heavy wrenches, resetting the die and rollers on Mill #3. Employee strained upper & lower back.
Ee was trying to pull out a piece of belting when he felt pain in groin area.
While walking, the employee struck his left knee on the bumper of a parked tractor. He called off work on June 17 and sought treatment on June 24. A physician took the employee off work for therapy. Filing of this 7000-1 was pending receipt of medical documentation.
EMPLOYEE BENT OVER TO MOVE MINER CABLE - FEELING SHARP PAIN IN HIS LOWER BACK. (CONTINUED TO WORK). WENT TO TO DOCTOR ON 1/19/05 AND 1/21/05. STARTED MISSING WORK ON 1/22/05.
Ee stated that he was going to set the remote box for the miner on top of the miner and adjust the carrying strap when a piece of rock fell from the roof between the roof bolts. The rock struck him on the right arm. Ee was transported to the hospital where it was determined that he had a fractured arm.
Ee was hurrying and stumbled stepping on the edge of his foot. He fractured the bone on the outside edge of his left foot. Ee is off of work until injury heals
Employee was pulling miner cable and felt pain in lower back. Started missing work 6/14/2004.
Employee strained his shoulder on 6/12/04, but didn't seek medical treatment or miss any work at that time. On 3/22/05, employee reported that his shoulder was still hurting, so he saw a doctor on 3/25 and had surgery on 4/18. We did not receive medical records to determine whether this was an illness or an injury until 5/10/05.
HE WAS PUTTING IN 2 10"" PIPES AND PLACING THEM TOGETHER SO ANOTHER EMPLOYEE COULD TACK THEM TOGETHER. WHEN THE OTHER EMPLOYEE STRUCK AN ARC, THE CURRENT WENT THRU BOTH ARMS FROM FINGERS TO ELBOW BEFORE EE RELEASED.
EMPLOYEE WAS ROOF BOLTING ON UNIT 3. HE WAS INSTALLING A ROOF BOLT INTO THE HOLE, THE ROOF BOLT LODGED CAUSING IT TO BEND & STRIKE HIS RT. HAND & WRIST. THE INCIDENT CAUSED A FRACTURE TO THE RT. WRIST. THIS INCIDENT IS UNDER INVESTIGATION PENDING THE DR.'S REPORT. EE MUST KEEP HANDS CLEAR FROM PINCH POINTS.
As the shearer was mining from the tail to the head a piece of rock four to five inches in diameter fell from the roof, hit the tail drum, & was thrown. The rock struck injured in the nose & forehead, breaking his glasses & the bill of his hard hat & causing a 1-1/2 inch laceration on the bridge of his nose.
EMPLOYEE WAS USING AN ELECTRIC HAND GRINDER ON A LOADER BUCKET. IN THE PROCESS OF GRINDING, THE GRINDER BLADE KICKED BACK ON HIM AND STRUCK HIS LEFT KNEE. THE RESULTING LACERATION WAS WASHED OUT AND STITCHED AT AN EMERGENCY CLINIC.
At the end of his shift he had completed taking a shower, as he was exiting the shower stall, he tripped on a ledge used to keep water in the stall. He fell onto his right knee and twisted his back.

Ee fell from the step landing on the service truck. Ee broke his fall with his hand resulting in a fracture to his small finger on his left hand.
Injured was struck by a piece of roof rock measuring 83"" long x 31"" to 13"" wide x 8"" to 9"" thick. The rock fell from between the rib and bolts without warning. Roof conditions appeared to all be good in the area. Bolt spacing was in compliance.
EE WAS HELPING PLACE A TOP ROLLER ONTO THE BELT STRUCTURE. HE REPORTED THAT AS HE LIFTED THE ROLLER AND TRUNED TO PLACE IT, HE INJURED HIS BACK MUSCLES.
EE HAD REMOVED GOOD ROCK FROM SAW BAY, LEAVING END PIECES STANDING. HE THEN REMOVED ANOTHER STACK OF GOOD ROCK, HITTING THE SCRAP ROCK CAUSING A DOMINO EFFECT WHILE STANDING AT THE END OF THE LAST ROCK AND IT FELL OVER STRIKING HIS LEFT FOOT. THE CRANE IS CONTROLLED REMOTELY BY EE. HE LACERATED THE TOP OF HIS FOOT WITH DEEP CONTUSIONS.
EMPLOYEE WAS PREPARING TO INSTALL A ROOF STRAP WHEN A PIECE OF ROOF 25""X 25""X 3""THICK, FELL OUT BETWEEN BOLTS, STRIKING HIM ON THE HEAD.
