On weekly examination-there was a roof fall found int he #7 intake entery at break #19 10' inby spad #462 in a timbered off area, the fall was approx 7-8' high and about 40' long, it does not have ventilation on intake travelway blocked it may a may not be celaned up, fall was found at about 730 am
An unintentional roof fall occurred along the 2A beltline in the #5 entry at the #11 crosscut. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 35' long x 18' wide x 10' high. The roof fall was cleaned up and the area was re-supported.
LOCATION IS SPAD #113, SECTION 001 APPROX 2000' FROM ACTIVE FACE IN THE BELT ENTRY. VENTILATION OR PASSAGE OF MINERS WAS NOT AFFECTED. ALL AGENCIES WERE NOTIFIED.
FRICTIONAL IGNITION OCCURRED ON THE NORTH 004 SECTION APPROX 90' IN THE #9 ENTRY BETWEEN BLOCK 37 & 38. OPERATOR SAW AN ORANGE BALL THAT STARTED APPROX IN THE CENTER OF THE CUTTER HEAD & SPREAD TO THE RT END. BALL LASTED APPROX 3 SECONDS. THERE WAS NO POP JUST SOME SMOKE. NO CHARRING WAS FOUND & THE INVESTIGATION DETERMINED THAT BITS STRIKING THE SANDSTONE CAUSED THE INCIDENT.
An unintentional roof fall occurred at the Millennium Portal at the 7th North Longwall #1 Bleeder Entry at about 575East, 2919North. The fall was approximately 30' long, 18' wide and 8' thick. The fall did not affect ventilation or the passage of people. The fall will not be cleaned up.

SOME 8' BOLTS ALL RESIN FULLY GROUTED. THE AREA WAS DEVELOPED IN JAN. OF 2004 AND WAS 55 FT LONG, 20 FT WIDE, 6 FT HIGH. 9000 FEET FROM WORKING FACE.

THE 5 HP MOTOR ON PRESSURE TANK WENT TO GROUND. HAD TO CHANGE MOTOR. THIS HELPS OPERATE BRAKES. THE MEN UNDERGROUND WERE NOTIFIED & PRODUCTION WAS STOPPED. THE EMERGENCY HOIST IN SHAFT WAS SENT TO BOTTOM. THE HOIST WAS RUNNING AGAIN AT 11:00 AM. IT WAS DOWN 1 HR 45 MIN.
THE CEDAR CREEK ELEVATOR WAS OUT OF SERVICE ON 8/30/05 AT 7:55 AM DUE TO A PLANNED SHUT DOWN OF THE POWER CIRCUIT WHICH CONTROLS THE ELEVATOR SO THAT REPAIRS COULD BE MADE TO A POWER POLE CURCUIT. POWER WAS RESTORED TO THE ELEVATOR AT APPROXIMATELY 10:00 AM.
Due to hurrican Katrina we received 3.5"" of rain in an hour and a half. At 1:30 pm due to rising water the section was notified to evacuate the mine. All miners were out of the mine by 2:00 pm. At 2:15 pm water started entering our return portal and was 6"" to 8"" at 1xc. Pumps kept the water receeding from the return portal. Second shift started in the mine at 4:15 pm.
A roof fall occurred in an outby area 2 crosscuts inby station 2628 in #4 entry of 8 Mains running across to #8 entry. 20' wide x 260' long x undetermined height.
A roof fall was discovered at the 48th crosscut of the 5 west mains panel in the #3 entry. The fall occurred in the belt entry. The fall is approx 30' l, 20' w and 7' h. The area had been bolted with 5' high Teck bolts, and spotted with 12' bolts.
An unintentional roof fall occurred in the crosscut between entries 5 & 6 at the #47 crosscut on the 3rd East Main. The area had been bolted with 6' fully grouted roof bolts.

Production hoist outage. Contractors installed new steel in the production hoist headframe, leaving little clearance. Positioning magnet on the west skip contacted the steel and was broken out of place, preventing the hoist from operating in auto. Mine evacuated at 8:20 a.m. and returned to work at 1:30 p.m. MSHA field office contacted.

A roof fall was discovered at 11:30 am on 8/29/05, the fall was located 30' inby s.s. #5346, and it measured approx. 4' high, 20' wide, and 60' long.
A roof fall occurred on the 4WB at crosscut 131 in entry #5. The cause of the fall was weak laminated shale. The fall was located inby the tailpiece for the 4WB belt.
A SMALL ROOF FALL WAS DISCOVERED AT THE MAIN PORTAL ABOUT 30' FROM THE SURFACE. THE FALL CONTINUED INTERMITTENTLY UNTIL ABOUT 40' OF ENTER HAD BEEN FILLED WITH LOOSE MATERIAL. CASTLE DALE, UT, MSHA OFFICE WAS NOTIFIED AT 8:00 AM. ALL EMPLOYEES WERE ACCOUNTED FOR AND A RECOVERY PLAN WAS MADE ADN POSTED ON BOTH ENDS OF THE CAVE.
An unintentional roof fall occurred in the # 6 neutral entry along the # 8 belt at crosscut # 3. The fall measured 25' long, 19' wide, and 5' thick. The fall occurred due to coal streaked top. The type of roof supports which was used in the area was 5' torque tension roof bolts. The area was developed May 2005 and was 3300' from the working face.
A ROOF FALL WAS DISCOVERED ON 8/29/05 ON 2ND SHIFT FIRE BOSS RUN ON #2 BELT ON NORTHWEST MAIN. 30'X20'X6' NEAR SS#96 IN A SECTION ENTRY
A roof fall was discovered at the mouth of the intake portal at approximately 9:30 pm. The fall measured 10 to 12 feet high, 55 feet long and 18 feet wide.
A fall of ground occurred at the 751 stope and 37 decline intersection. A portion of the fall extended beyond the installed 6' & 8' roof bolts. The failure plane was noted to be a result of a thrust contact between argillite and phyllite rock types. Corrosion of the support may have contributed to the failure. Approx. 160 tons of material fell.

An unintentional roof fall occurred at the Galatia North Portal at 4108' in the #2 entry of the Flannigan Unit. The fall was approximately 30' long, 30' wide and of unknown thickness. The fall did not affect ventilation or the passage of people. The fall will be cleaned up.

5:15am Ee noticed 79 x c along main east track was taking weight. He then knocked the trolley power and called tex rader acting shift foreman and informed him of the condition. rader then sent a man to guard the area from 8 blocks away until rader arrived.He stated that at 6:17 am the intersection at 79 xc 82 + 68 on main east track fell. the fall was approx. 20'lx16;wx13'h.
Roof started working #2 Entry, from the face back in 1 Rt Section back 2 XCut. Roof started to sag and then fell.

A roof fall occurred on 1st left in no. 5 entry no. 8 break with no restriction to ventilation or travel. The fall will be cribbed off and not cleaned up.
A roof fall occurred on the old main line at the Cook's Branch Portal. The fall was approximately 6' high x 20' long x 20' wide. The fall will not be cleaned up and the area was breakered off with cribs. The fall was discovered by the evening shift.
A 4"" rain fell in a 2 hour span; we had trouble with the barge pumps in the pit and as a result water inundated our #1 slope portal. We have since re-routed some ditches and added another pump to the barge to avoid a re-occurrance of this accident.

BN Train that was on the main track started a grass fire, that lasted more than 30 minutes. Area that was burned was strip of grass between the main line and Dry Fork Mines rail -loop. No damage to structures or equipment. No injuries. the only damage was burned grass.
Roof fall in abandoned area of north side of the mine. Area had been previously been barricaded off due to bad roof conditions.Fall was at intersecton of 3A & S16 . Secondary escapeway was inspected and found secure .

The #5 entry was driven in approx. 90'. Last 15' to 20' was unbolted. Midnight shift had dangered it off & reported that add'l bolting needed to be done outby the cut. Day shift was installing 10' bolts with straps 30' from fall area before it started working. Entire entry had been bolted with 5' & 6' full grouted 5/8 resin rods. Fall was approx 60' long x 10' high x 18' wide.

A roof fall measuring 3 feet by 18 feet by 20 feet was found at the end of the #1 belt system. The roof fall is located approximately 400 feet from the mine portal and was supported with 36 inch resin bolts.

The Galatia North elevator became hung in the shaft due to lilly controllers being out of adjustment.
An unintentional roof fall occurred at the Galatia North Portal slope at 960'. The fall was approximately 8' long, approximately 16' wide and approximately 4' thick. The fall did not affect ventilation or the passage of people. The fall will be cleaned up.

Roof fall at spad #1332 in #1 entry return. Approx 6' thick x 20'. All access has been timbered off & return has been routed around the fall. Area was bolted with 4' resin bolts (fully grouted) with some 6' tension rebar bolts spotted.
Electrical service power from provider was off due to electrical storm. 2 other hoists were available for miner evacuation. Power was restored at 4:00 am on 8/20/05.

Their was not any injury(s) or equipment damaged nor did it impede ventilation. The roof broke above the bolting horizion. Location 29 South @ 17 East. Size of fall is approximately 50ft x 50ft x 14ft. This area was blocked off to all mine personnel since 5/24/04 anticipating this fall.

AN UNINTENTIONAL FALL OF ROOF OCCURRED DURING RETREAT MINING AND CAUGHT THE CONTINUOUS MINER INBY BRIDGE AND INBY BRIDGE CARRIER, ONE BREAK OUTBY SPAD #3370. THIS OCCURRED ON 002 SECTION
A FALL OCCURRED IN THE #3 ROOM OFF 3E ENTRY'S ON 003 UNIT AT THE 0+65'. FALL WAS 25' LONG X 18' WIDE X 8' LONG.
Roof fall on intake inby spad 2890 in #2 entry. Intake air was not blocked and no employees were injured. The area was dangered off and timbers were set.
This is not a fall, it is just a little sloughage from a previous fall that happened on February 27, 2005, in the 5North Headgate between Entries 2, 3 and 4 at Crosscut 53458 North. It is of unknown length/width/thickness. It did not affect ventilation or passage of people.
A reportable roof fall occurred in E-East Mains, entry 2, crosscut 12. Fall was approximately 7 feet high, 20 feet wide, and 30 feet in length.
An unplanned roof fall occurred outby 003 section measuring 20' x 20' x 4' thick. The fall was located in #6 entry at Spad #4525. Fall was timbered, cribbed and dangered off. Roof was previously supported with 4' resin bolts.

An unintentional roof fall occurred in 5H section at #9 wall, #2 entry. The fall measured approx. 16' W x 30' L x 8' high. No injuries or equipment damage.
An unintentional roof fall was discovered in the #4 entry of grassy creek mains at survey station # 22023 approx 4,500 ft from the nearest working face. Ventilation was not disrupted, no machinery was involved and no injuries resulted from this fall. Size is approx 20' wide by 30' long by 10' high. Fall will be cleaned up.
An unintenitional roof fall occurred 9 x-cut, #3 entry 10 lt north maijs, the fall measured 20'x20'x4-5'.
A roof fall occured in the #10 intersection of the #4 heading on the 8 West section. The fall was approximately 20 feet wide and 20 feet long and ranged from 0 to 9 feet thick. The area was bolted with 6 foot install b bolts.
North 1, 38 block, 1 entry seal. 30 ft long x 40 ft wide x 12 high. Support: 6 ft 3/4 inch resin bolts, roof mats, cable truss bolts, ACS jacks, cribs. Fall dangered off, additional roof support installed around fall.
High speed bearing went out on elevator drive. Will be out of service for 2 days until repairs are made. Man hoist as well as emergency hoist will be manned and ready 24 hours a day until elevator is returned to service. A message was left at the Lexington Kentucky MSHA office on 8/16/2005 informing of the outage.

An un-intentional roof fall occurred along No. 5 belt near survey station 3650. The fall was app. 50 feet long,20 feet wide, and 5 feet thick. The area was bolted with 4 foot resin bolts. The fall will not be cleaned up, the section will be relocated outby this area.

A rock fall occurred on #1 unit, #8 entry intake. The fall was 80 x 18 x 5 1/2.
A roof fall 80 feet long, 10 feet thick, 20 feet wide occurred on 8-16-05 @ 12:30 a.m. in number 1 entry.
The NEMA baghouse had an unplanned ignition resulting from smoldering coal being sucked into the baghouse as the baghouse fan was turned on after a pro-longed outage. The ignition resulted in one of the explosion doors being blown open. The system was shut down, MSHA was notified and the hot spot was eliminated by the use of water.
An unintentional fall of roof was discovered during the weekly exam of return airways. The fall was located on Elk Creek Mains #15 break #4 entry.

HOISTMAN PUSHED BUTTON TO SEND MANCAGE TO SHAFT BOTTOM. WHEN THERE WAS A POWER BUMP, THE HOIST STOPPED. THE HOISTMAN RESET THE SURFACE BREAKER THAT HAD TRIPPED. WHEN HE TRIED TO RESET THE HOIST, IT WOULD NOT. THERE WAS A COMPONET FAILURE, ELECTRICAL SUPERINTENDENT WAS CALLED OUT
#2 man and material hoist experienced a malfunction that caused the hoist to be out of service for more than thirty minutes. an electrical connection had grounded due to water from the washing the shaft. It was identified upon investigation and the hoist put back in.
A roof fall occurred on the 1C panel, at crosscut 44 in entry #1. The cause of the fall was weak laminated shale. The fall was located in the return of #1 unit and fell in the weekly examiners route of travel.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE GALATIA NORTH PORTAL AT 3880' IN THE TAILGATE ENTRY OF THE 12 WEST LONGWALL. THE FALL WAS APPROXIMATELY 12 FEET LONG, APPROXIMATELY 18 FEET WIDE AND OF UNKNOWN THICKNESS. THE FALL DID NOT AFFECT VENTILATION. THE FALL WILL BE CLEANED UP. FALL #22TT.
Slope hoist had electrical problem, leaving hoist inoperable. Electrical problem was repaired on 8/12/05. Bottom slope doors were repaired on 8/15/05 and slope hoist was put back into service at 8:00 a.m.
The elevator quit working due to going up into the overtravel stops. The PLC would not allow the elevator to back out of this top stop. It was found that dirt got in a contactor. The elevator was done for more than 30 minutes but had no damage. Elevator was back running at 3:30 pm.

A truck driver noticed smoke in the exhaust compartment of his truck when he was at an ore dumpsite. He found some small flames and extinguished the fire. He thought it was out. Other ee saw smoke in the ramp so he evacuated the mine. Mine rescue personnel found the fire. 2 rags had caught fire. Fire lasted app 2 minutes.
THE LAKESHORE HOIST AT THE ELKHART PORTAL WAS OUT OF SERVICE FROM 1 PM UNTIL 11 PM ON AUGUST 10TH 2005, FOR REPAIR OF EMERGENCY BRAKE DOG ASSEMBLY. REPAIRS WERE MADE, HOIST INSPECTED AND WAS THEN PUT BACK INTO SERVICE.

VENTILATION WAS NOT IMPAIRED AND NO INJURIES. MINING WAS TEMP STOPPED AND IMPLEMENTATION OF THE APPROVED BLOCKED TAILGATE PLAN WAS PUT INTO AFFECT PRIOR TO RESUMING MINING

On 8/4/05, an unintentional roof fall occurred along the 3rd south, 1st west sub-man at cross cut #34 int he #2 entry. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 30'l x 20'w x 12'h and the roof fall will not be cleaned up.
A roof fall occurred in the No.1 entry of the 1-left off 1 right mains section at survey station 4629 (40 feet outby). The fall was approx. 25 feet long by 12 feet wide by 4 feet thick. The fall injured 2 persons who were involved in a rescue and recovery operation. In addition the fall caught a scoop which was being used in process.

As employee was eating lunch in the pump house, a truck hit pump house causing fatal injuries.
Truck rolled into pump house where employees were eating lunch. Employee was fatally injured.
Employee was clearing trees on top of an embankment with a caterpillar d5m lgp bulldozer when the bulldozer went over the embankment coming to rest on its side.
While employee was driving the haul truck, it appears he lost control of the vehicle and went over a 7 - 9 foot high berm into the lake that was approximately 70 - 80 feet deep.
A piece of rock fell from between the pin & rib striking injured on head, shoulder & back. Victim died 11/15/05.

Employee bent down to pick up a bucket weighing approximately 10 pounds and felt a pop and experienced pain in his right groin area. Medical examination revealed a small hernia.
Employee was cleaning chopper. He was holding up the table to clean underneath and the table slipped smashing his left hand. His middle finger and ring finger tips were severed. Upon hospitalization, the middle finger was re-attached.
WE WERE REMOVING THE CHEEK PLATES OUT OF THE JAW TO TURN THE DYE IN THE JAW AND ONE OF THE PAD EYES BROKE OFF AND STRUCK EMPLOYEE
Boom pin bushings were being put in miner head and the miner head slipped and caught the tip of employee's left middle finger between the bushing and the main frame. Resulted in amputation of distal phalange just below fingernail. No other fingers injured.
This employee attemped to move and set a shield by remote means. The shield appears to have set by the auto-set function. The side seal was over lapped and the side seal came off striking the employee.
Employee was preparing to switch product gates at the final wash screen in preparation of running a different product. Employee pulled on handle of product gate and found it difficult to move as material had lodged. The employee used force on handle causing strain to lower back.
Loaded shuttle car was leaving miner. Injured was working as miner helper. He stated he tripped and fell. Left leg went under rear bumper or was struck by bumper causing compound fracture and laceration of lower left leg. Physician determined to surgically remove lower leg.
A nylon rope was being used to pull a cable across the pond for the purpose of loading a larger cable onto the yarder that would drag the bucket. Rope snapped & wrapped around employee's arm.
A BELT MECHANIC WAS STANDING ON A LADDER WHEN HE SLIPPED OFF THE RUNG. HE FELL TO THE MINE FLOOR & INJURED HIS RT. INDEX FINGER (LACERATED), RT. SHOULDER (CONTUSION) AND HIS RT. LEG (STRAIN). HE CONTINUED TO WORK UNTIL SEEN BY A DR. AFTER HIS SHIFT ON 09/27/05. HE WAS TAKEN OFF WORK & BEGAN MISSING TIME ON 09/28/05.
HE WAS RUNNING ROOF BOLTER HE TURNED AROUND TO GET A ROOF BOLT OFF RIB AND TWISTED HIS KNEE
EMPLOYEE WAS INSTALLING RIB BOLT. HIS GLOVE GOT HUNG ON BOLT, TWISTING HIS GLOVE OFF DISLODGING HIS 4 AND 5 FINGERS ON LEFT HAND. SURGERY WAS REQUIRED TO RESET FINGERS WITH SUTURES.
Employee was walking up belt line packing a shovel and dinner bucket and was stepping across rib roll when he slipped and fell and hit wrist on slate rock.
TWO EMPLOYEES WERE TRYING TO GET PUMP PRESSURE FROM GLUT TO FACE AREA. INJURED WAS TURNING #3 VALVE ABOUT 1/4 TURN WHEN PRESSURE CAUSED GLUT TO BREAK CHAIN THAT HELD GLUT TO MONORAIL. GLUT KNOCKED BOTH TO THE GROUND. EE SAID GLUT HIT INJURED IN THE HEAD. X-RAY SHOWED FRACTURE IN NECK. ALSO RECEIVED SUTURES.
Employee was helping to change batteries on scoop when the battery fell off of a prop hitting him on the left knee.
THEY WERE DISCONNECTING 6"" WATER PUMP HOSE - THE WATER WAS NOT PUMPING; WATER GOT HOT. WHEN THEY DISCONNECTED THE DISCHARGE HOSE AT THE BOTTOM END, 20 FT. FROM THE PUMP, WATER CAME OUT AND BURNED EE ON HIS MIDSECTION, LEGS & ARM. SKIN GRAFTS REQUIRED ON ABDOMEN & ARM.
EE KNEELED DOWN AT #2 HEAD STARTER BOX, TROUBLESHOOTING. ROCK FELL FROM TOP AND STRUCK EE ON SHOULDER AND BACK OF NECK.
Moving miner cable out of road. Road was mudey he sliped and fell he worked til 12-14-05 went to doc. and he took him off work. Was not reported to me til 12-20-05.
A Euclid haul truck was coming out of the pit. The driver looked down & the truck veered onto the wrong side of the road. EE was driving a Mack truck, going toward the pit. When EE saw the Euclid coming at him it was too late to avoid a collision. EE's right shoulder & left ankle were sprained and bruised. The driver of the Euclid was not injured.
EE was climbing down a ladder on an IT28 (loader) and strained his right ankle.
Employee reported that he was in the shower when a toilet was flushed, the water turned hot and he jumped from under the shower and slipped and caught himself with right arm. Employee reported shoulder pain.
Employee was shoveling when he strained his right wrist. This was orginally reported back on 8/31/05 as a near miss but waited till the 10/20/05 to request to go see a doctor. This is when the treating physician gave the employee light duty restrictions.
Employee was driving a water truck. The truck turned over. Employee reported bruising to both arms and the middle of his back.

Truck driver was pulling down roadway into pit and hit his head on roof of truck from a bump in roadway, hurting his neck.
The pump blocked up and he went to unblock it. It was hydrating inside pump and it blew up in his face giving him burns from the lime in both his eyes.
2LEFT-5SOUTH LONGWALL RECOVERY. MAN HAD ATTACHED HOOK TO A FACE SHIELD-ATTACHED TO CHAIN AND HOOK TO SHIELD TO BE MOVED. THE SHIELD TO BE MOVED SLIPPED OFF EDGE OF SCOOP CAR DUCK BILL. THIS CAUSED HOOK TO COME LOOSE AND HIT EMPLOYEE
HE WAS SPLICING A CAR CABLE WHEN A RIB ROLLED OFF AND STRUCK HIM IN HIS LEFT LEG.
Trying to pick up structure, he said it fell on left foot.
Mechanic was moving the jack pad on the drill when he started feeling pain in his lower back. Pulled muscle in lower back. Missed work from 9/6 thru 9/10/2005.

Member tripped over a flexible water hose at top of slope twisting his left knee.
EMPLOYEE WAS LIFTING A BOTTOM ROLLER AT #8 WALL CROSSCUT F-13 BELT TO TRAIN THE BELT AND FELT A PAIN IN HIS LEFT GROIN. EMPLOYEE CONTINUED TO WORK. LATER DIAGNOSED AS A HERNIA AND BEGAN MISSING WORK ON 11/1/2005.
WHILE STEPPING OF LOADING DOCK, EMPLOYEE LANDED ON A PIECE OF CONCRETE AND SPRAINED LEFT ANKLE.
EE was holding a blunt-end chisel while a coworker struck it with a hammer. The coworker missed the chisel head and struck the EE on his right hand, fourth finger. Result was a laceration and a fractured finger. Placed on restricted duty.
OPERATOR WAS EXITING FRONT ENDLOADER PROPERLY AND SLIPPED AND FELL OFF STEPS TO THE GROUND. SLIP WAS CAUSED BY OIL ON STEPS FROM FILLING HYD. TANK & OPERATOR WAS WEARING BOOTS WITH SMOOTH SOLES.
Loading supplies on roof bolter. When loading straps on bolter, 1 man was sliding straps on stack. Employee's hand was stuck and cut left hand.
EE WAS IN THE PROCESS OF ROOF BOLTING PROCEDURES IN THE #6 HEADING ON THE #004 CONTINUOUS MINER SECTION. A PIECE OF DRAW ROCK (APPROX. 29"" X 30"" X 3"") FELL ON TOP OF THE CANOPY, THEN SLID OFF THE CANOPY HITTING THE EE ON TOP OF THE RIGHT FOOT CAUSING A FRACTURE OF THE FOOT & ANKLE. THE FRACTURES WERE NOT FOUND UNTIL A BONE SCAN WAS DONE ON 10/3/05.
EE was driving off road truck and backed under the backhoe bucket to get loaded he did not stop when the bh operator blew the horn and the bh operator had to stop the truck with the bucket. He said this made his back hurt.
After completing a belt move, Injured and another employee were palnning to build an intake stopping.Helper set the bucket down on the 35-c scoop pinching injured's right foot at the tip end of the metatarsal. Only the 2 smallest toes on right foot were bruised.
Employee was helping to lift an approximately 100 lb stone into pickup truck and strained a lower back muscle
Employee was using a battery powered cart to deliver a part to his jeep. After unloading the part, the battery was out of power. He pulled the cart out of the way and felt pain in abdoment. Diagnosed with hernia and lost work days began 9/7/05.
While moving a length of miner cable, employee felt sprain to left shoulder.
Employee was assisting three others in replacing accuator arm using chain hoists. One man let go of his end and accuator fell on employee's shoulder and neck causing muscle strain. This was not reported to office until 9/12/05 when employee stopped work due to pain from this injury.
Injured and co-worker were putting a slider tube inside a 20 inch ventilation tube they had just hung. The coal rib behind the tubing fell and struck him on the left leg above the knee. He began losing work the next scheduled shift.
Leaving at quitting time - EE was walking up a grade to his car at a fast pace to get out of the rain when he slipped in some mud and fell.
EMPLOYEE WAS PUSHING BLOCK THROUGH THE CHOPPER WINDOW WHEN HE FELT A PULL IN HIS BACK. HE WAS WEARING A BACK SAFETY BELT.
While removing #5 kiln tripper screen and replacing skirting and tracking belt, they got lime on their torso causing a burn.
Scoop stuck, took tractor to pull scoop out, stoped tractor close to scoop got off between them, tractor rolled down in small hole and caught leg between it and scoop.
Forklift operator was putting tarp on top of loaded trailer. Tarp was heavier than usual because of rain and mud. He twisted his left wrist.
INJURED WAS WALKING TO PUT UP TOOL IN TOOL BOX WHEN HIS FOOT CAUGHT IN SOME SCREEN WIRE LAYING ON THE MINE FLOOR. THIS CAUSED INJURED TO FALL AND BRUISE HIS LEFT LEG.
EE WAS OPERATING RT. CONT. MINER IN THE #5 ENTRY & RIBBED OUT THE STANDOFF DAMAGING THE CABLE & KNOCKING THE BREAKER AT THE POWER CENTER. ATTEMPT TO RESET THE BREAKER, POWER OUTSIDE. ELEC. WAS INFORMED OF THE SITUATION, EXAMINED THE CABLE & TRAVELED TO THE POWER CENTER TO DE-ENERGIZE THE CIRCUIT & BLOCKED & TAGGED THE WRONG CATHEAD. POWER WAS RESTORED & HE GOT ELEC BURNS
Employee was working on suction on offside of bolter when the operator side operator came around to offer assistance. The operator from the operator side leaned over to help, hitting the lever causing the head to go up, catching offside operator's fingers between the head arm and the arm going to the feed jack. He suffered lacerations and fractures to fingers on his left hand.
Employee was using a water hose to do cleanup at changeover. When plant valved down sand side of plant, the water pressure increased causing employee to pull a muscle in his back.
EE STATED HE WAS LIFTING FENCE GUARDS AND CARRYING THEM TO THE SCOOP BUCKET WHEN HE SLIPPED IN MUD AND TWISTED HIS LEFT KNEE. AS THE SHIFT CONTINUED, HIS KNEE BEGAN TO SWELL.
EE WAS COUPLING TWO DRILL STEELS TOGETHER WHEN THE STEEL BENT AND CAUGHT HIS RIGHT THUMB BETWEEN THE STEEL AND THE TRS JACK, CAUSING AN AVULSION OF THE LOWER PART OF HIS RIGHT THUMB.
EE WAS ATTEMPTING TO LIFT A TORQUE CONVERTER THAT WAS PARTIALLY BOLTED INTO A 992 LOADER. THE PICKING EYES BROKE CAUSING THE CRANE BLOCK TO JUMP SEVERAL FEET IN THE AIR AND ONE OF THE CABLES TO COME PARTIALLY OFF THE SHIV. HE DID NOT NOTICE THE FAULTY SITUATION & CON'T HIS JOB ABOUT 3' UNDER THE 200# BLOCK. THE BLOCK DROPPED ONTO HIS HARD HAT PINNING HIM AGAINST THE LOADER.

Employee was in process of unloading an ITE box at the mouth of 9A longwall section on the supply track entry. While lifting he felt pain in right lower back. Began missing on 8-30-05. Was informed by employee diagnosed with strain - experiencing muscle spasms.
Employee was removing the belly pan from a D-11 dozer when it fell on his legs. He was instructed to place the tractor on our concrete pad and use the designated JTA procedures for removing the pan. He was told to use a jack and to be sure to be careful in the process. He decided to start cutting the pan loose from the tractor and had no cribbing or jack under the pan.
EE HAD HEATED A FINAL DRIVE BOLT TO BREAK LOOSE WITH 1"" IMPACT. ALSO, HE HAD PUT BLASTER AIR TOOL OIL IN IMPACT. WHEN OIL CAME OUT WITH AIR PSI FROM IMPACT IT IGNITED AND BURNT HIS LEFT FOREARM, RIGHT HAND AND LEFT SIDE OF HIS FACE.
BAP OPERATOR CONTACTED HOT LIQUOR WHEN THE SUMP PUMP GRATING HE STEPPED ON FAILED. HE SUSTAINED BURNS 2ND/3RD DEGREE TO LEGS, FEET, ARMS AND BACK
Employee was cutting coal with Joy miner and a piece of metal broke off of bit when it hit sandstone in the coal, metal flew back about 50' and struck him in the right eye (safety glasses were provided but he was not wearing them).
Employee was working over time shoveling on the conveyor belt. While shoveling a rock fell from the roof and struck him on the wrist and forearm.
EMPLOYEE WAS TAKING SHUTTLE CAR CABLE AROUND THE CAR. HE HAD HIS HAND ON TOP OF CAR AND A PIECE OF DRAW ROCK FELL STRIKING HIS HAND
Employee was climbing down a ladder on a FEL. His left foot slipped off the bottom step, with his knee catching the step, causing his knee cap to push upward. Injury is inflammation of the left knee cap area.
Employee was changing the da ram on the longwall and used improper lifting technique.
Employee dismounted machine at end of shift and stepped on a large stone twisting his right ankle.
HE WAS WALKING BELT HE STEPPED IN A HOLE TWISTED HIS RIGHT ANKLE AND LEFT KNEE.
In the course of performing his normal duties, employee thinks he twisted his left hip while ascending and descending the equipment.
Roof bolter operator was looking up where he was drilling a hole in the roof and something flew from the bit and roof and went behind his safety glasses. Cause of this injury was habits. Compliance with rules/regs was not a factor, protective equipment was ok, miner proficiency a factor, mining equipment/systems not a factor.
Employee was repositioning an 8' step ladder inby 3 North #4 belt drive to assist with removing a bad bottom roller. While lifting the ladder out of the mud he felt a pain in his lower back.
Employee bent down to put crib under miner and when he stood back up his knee popped with pain. He finished his shift but next day knee was swelled so he went to doctor and returned to work next day.
On 8/17 EE fell to ground while boarding truck. Slight pain in back, dr gave him dose of pain med, released to work. On 8/28 he told his supervisor he'd hit pothole previous day, aggravating back. Went to dr, given full release. Worked reg. schedule till 9/11, called off w/back pain. On 9/12 notified supervisor dr had taken him of work for 30 days.
Employee was climbing onto drill when his foot slipped on step resulting in a pulled muscle to the left shoulder.
Employee was geting out of the scoop to close the airlock doors. He slipped in the mud with one foot in the scoop and one foot out. When he fell he hit his groin area on the scoop frame.
ON 8/26/05 EE WAS PERFORMING SERVICE WORK ON A CONTINUOUS MINING MACHINE. HE WAS IN THE PROCESS OF ADDING OIL TO THE POTS WHEN A PIECE OF ROCK (4' X 6' X 2"" THICK) FELL OUT FROM BETWEEN THE PERMANENT ROOF SUPPORTS, STRIKING HIM ON THE TOP OF HIS RIGHT FOOT.

Operating Fletcher bolter in face area, when attempting to remove the drill steel from the machine's rotation chuck, experienced pain to his left lower back region. Request comp. carrier to request all pertinent medical information. Mine was notified 9-13-05 ee was diagnosed with degenerative disc disease to his lower spine region. Mine charging as an illness - lost time.
Employee was attempting to remove 7 roof screens from a bundle of screens that were leaning against the rib when the entire bundle tipped over striking ee in the left leg.
Employee lost his balance and jumped from D9 dozer walkway resulting in a fracture to his right heel.
Operator was dismounting truck, the bottom cable step slipped and caused his left foot to strike the ground very hard. It had been raining & there was a lot of mud. His left knee was dislocated and will require surgery.
Employee was plant oiler at the time of injury. He stepped out of the oiler truck twisting his left ankle on August 26 2005.He has continued his regular work duties until April 25 2006 when is Dr. removed him from all work activity for 30 days. At the end of 30 days he will be reevaluated.
EE WAS WALKING TO SLOPE BOTTOM. SOMEONE HAD ATTEMPTED TO CLOSE THE AIRLOCK DOORS. HE TRIED TO HURRY THROUGH THE DOORS BEFORE THEY CLOSED. HIS COAT SLEEVE CAUGHT ON THE DOOR; RESULTED IN HIS ARM BEING CAUGHT BETWEEN THE CLOSING DOORS. EE WORKED ALL SCHEDULED SHIFTS UNTIL 9/7/05, THE DATE HE BEGAN MISSING SCHEDULED WORK DAYS. BECAME REPORTABLE ON THAT DATE.

Employee was installing a 42""resin bolt in the roof. As he was pushing the bolt up the bolt bent & sprung out of the pot & the 8 x 8 plate cut his left forearm.
Employee was changing the tailgate leg when the bottom stage ring blew off, striking him.
EE WAS IN THE PROCESS OF FUELING A 575 DOZER. HE WENT TO STEP ON THE PUSH ARM AND SLIPPED, FELL TO THE GROUND AND TWISTED HIS RIGHT KNEE.
AFTER COMPLETING THE MINING OF #2 ENTRY, WHILE BACKING THE BRIDGES DOWN THE BELT ENTRY INJ EE PLACED HIS HAND ON THE LOWER REAR EDGE OF THE OPERATORS DECK DUE TO GETTING TOO CLOSE TO THE LOW STRUCTURE EE'S HAND BECAME CAUGHT BETWEEN THE LOW STRUCTURE AND THE BRIDGE DECK.
LIFTING A 5 GALLON BUCKET OF WATER WHILE PRIMING THE BANK PUMP FOR THE GRAVEL WASHER.
EMPLOYEE WAS ON #110 SHIELD AND #109 WAS PULLING. EMPLOYEE LEANED OVER CABLE THINKING THE SHIELD WAS THROUGH PULLING AND HE WAS MASHED BETWEEN THE TOP OF THE CABLE TROLL AND THE BOTTOM OF THE SHIELD. (Misc Mail 6/6/06 #1-employee lost time again for one day on 6/1/06)
The continuous miner was being used to push out a shuttle car. Employee was in the shuttle car and when the miner rode up on the car and pulled off, it bounced employee into the top of the cab. Lost time injury as of September 16, 2005.
HE WAS STANDING ON THE CAT WALK OF A BULLDOZER, WHEN THE BLADE WAS LET DOWN IT JARRED HIM AND HE FELL DOWN ONTO THE DOZER TRACKS.
EMPLOYEE WAS MOVING A TORCH TO BE USED AT ANOTHER LOCATION WHEN AN EMPLOYEE OF ANOTHER COMPANY (NATIONAL ELECTRIC) DROPPED A PIECE OF ELECTRICAL CONDUIT FROM OVERHEAD, HITTING HIM IN THE LOW BACK & TAIL BONE. DID NOT SEEK MEDICAL TREATMENT AND CONTINUED TO WORK WITHOUT RESTRICTION TILL 10/7/05.
EE stepped from the pipeline to the bank, sunk in the mud and injured his ankle. He did not begin losing time until 19 April 2007 when he had surgery on ankle.
Member was remounting a tee on a positive set block on the leg jack of a longwall shield. When he removed the staple from the tee, the tee and emulsion fluid spurted away from the leg jack and struck him in the right eye.
He was lifting the end of a Kennedy door and he felt pain in his lower.
Employee was hanging curtain in #1 Place and pulled his shoulder. IT WAS NOT DETERMINED THAT EMPLOYEE HAD A ROTATOR CUFF TEAR UNTIL 9/1/05 DUE TO DR REFERRAL AND MRI.
Was standing by an upright fuel tank the tank began to ran. He tryed to stop it and got is little finger trapped between the tank and the 55 gal drum the tank hit.

While reeling in the hose of the fuel truck, a kink in the hose uncoiled and the nozzle struck the employee's finger.

EE WAS CLEANING THE WINDSHIELD OF 992C LOADER WHEN HE SLIPPED AND TWISTED HIS RIGHT KNEE.
Employee was standing on bolter cable while it was tramming. Bolter turned and jerked cable and flipped him, hitting head. He continued working that shift and all scheduled shifts until he reported to work on 9/8/05 and told supervisor his head hurt and was dizzy and went home before shift started. This injury is being CONTESTED since he worked 2 weeks before having a problem.
Employee was bent over throwing a miner cable out of roadway when he strained his lower back.
EE HOPPED OFF FRONT TIRE SHE HAD BEEN STANDING ON TO PUT ANTIFREEE IN HER TRUCK. WHEN TOUCHING GROUND, HER BACK POPPED AND STARTED HURTING
Two employees were working on a belthead, preparing to take the slack out of the drive chain. When one lifted the top cover that guards the drive chain, he felt a sharp pain in his lower back.
Employee was walking to get a timber when a peice of rock 3'x5'x1' x 0 fell from the roof striking him on his left shoulder, back and left leg. Employee continued to work light duty until 10/25/2005, the date of his operation.
While operating loader, employee complained of numbness in leg and arm because the seat in the loader was bouncing too much.
Operating skid steer. Bucket latch not fully engaged and when EE dumped the load, the bucket fell to ground. EE raised his foot and was struck by falling bucket - chipped bone in foot.
Injured suffered a 2nd & 3rd degree burn to the palm and ring finger of his right hand when the piece of sheeting he was cutting out fell onto the oxygen hose and burned through it causing an intense flame which contacted his gloved right hand.
WHILE MOVING A GUARD THERE WAS A LOOSE PIECE OF WIRE THAT SPRANG BACK & STRUCK HIM IN THE BOTTOM OF THE LEFT EYE, CAUSING A FOREIGN B0DY TO THE LEFT EYE.
Employee had been cleaning the ditches in the dryer area, when he lifted the grates by hand he felt a sharp pain in his lower back and down his left leg. He was taken to seek medical attention on 8/25/05 and is scheduled for an MRI on 8/31/05. He will be off work until the result of the test is known.
THE EMPLOYEE WAS ASSISTING IN UNLOADING CRIB BLOCK OFF MATERIAL CAR WHEN HIS RIGHT INDEX FINGER WAS SMASHED BETWEEN 2 OF THE CRIB BLOCKS. RECEIVED 16 SUTURES.
Employee was changing roll plant screens. He scratched the tip of his elbow on the edge of the screen frame. An infection formed in his arm.
EE WAS IN THE PROCESS OF ASSISTING A SHUTTLE CAR OPERATOR ADD ADDITIONAL SLACK TO THE REEL OF SHUTTLE CAR. THE CAR OPERATOR WAS ADDING SLACK BY STARTING & STOPPING THE PUMP MOTOR OF THE SHUTTLE CAR. IN THE PROCESS OF ADDING SLACK, THE CABLE STRUCK INJURED IN THE KNEES KNOCKING HIM TO THE GROUND CAUSING A TORN CATRILDGE. HE CONTINUED TO WORK UNTIL SURGERY 10/21/2005
Exiting machine, his foot slipped on track. He managed to catch himself before falling to the ground. Someone had located a truck mirror on grab rail on cab; this mirror has been removed.
THE EMPLOYEE STATED HE GOT OUT OF THE NO. 6 MANTRIP TO ALLOW THE TRIP TO PULL UP A HILL AND WALKED TO GET BACK IN THE TRIP WHEN HE SLIPPED & FELL. HE HIT HIS KNEE AGAINST THE MANTRIP. THE EMPLOYEE HAD AN MRI SCHEDULED BUT DUE TO THE UNAVAILABILITY OF THE DOCTOR WAS NOT TOLD UNTIL 9/12/05 THAT HE WOULD NEED THERAPY & WOULD BE OFF WORK TO DETERMINE IF SURGERY IS NECESSARY.
While returning to the pit area employee fell asleep. Equipment crossed over a windrow, causing the operator to be knocked around withing the cab, causing injury to the employee's right side.
Employee was laying track and was flipping over a piece of track with a slate bar when he felt a pain in his lower back.
Employee was crawling to his next work position, as his left knee made contact with a stone on the walkway, his knee twisted in the knee pad he had on, straining the knee.
THE EMPLOYEE WAS OPERATING THE OFF STD 10SC SHUTTLE CAR WHEN HE HIT A POT HOLE THEREFORE BOUNCING HIM INTO THE CANOPY. HE THEN COMPLAINED WITH NECK PAINS
Employee was jacking a car on track & felt pain in lower abdomen. Lost work days began 8-26-05.
4I longwall section, employee was lifting a welder up on car and felt pain in right side lower abdomen. Diagnosed as hernia 9/6/05. Lost work days began 9/15/05.
PICKED UP THE HOOD OF A TRUCK TO CHECK THE OIL. STRAINED BACK WHILE LIFTING

Miner was climbing railcar, rung broke causing miner to fall.
EMPLOYEE WAS UNHOOKING A ROCK DUSTER FROM THE SCOOP BUCKET AND RECEIVED AN INJURY TO HIS LEFT ELBOW. THE TENSION ON THE CHAIN HOLDING THE DUSTER IN PLACE WAS TOO TIGHT CAUSING THE EMPLOYEES ARM TO HYPER EXTEND.
Due to an occupational illness as substantiated by Medical records, reports and The ee became unable to work because of ailments described in his medical records. This was not due to one signle event in the mine but repeated trauma overtime. sigler started missing work on 8/23/05.

Employee was washing down at T-2 tower using a 1-1/2"" water hose and said that he hurt his lower back pulling a kink out of the the hose.
Setting king beam leg, anchor bolt broke. Leg on beam kicked back & hit his leg causing bruising.
Employee was tightening bolts on a shuttle car wheel with a 3/4 drive ratchet and socket. The socket slipped off the bolt causing him to lose his balance and twist his right leg, causing a fracture.

Driver got truck out of gear going down hill, could not stop. Traveled to Mingo Logan property & turned over. Driver refused treatment on site. Later went to emergency room, treated and released to return to work.

EMPLOYEE WAS OPERATING THE CONTINUOUS MINING MACHINE. A PIECE OF ROCK SLID OFF RIB. EMPLOYEE MOVED OUT OF WAY OF ROCK, TWISTING KNEE. WENT TO DOCTOR ON 8-24-05. STARTED MISSING WORK ON 8-29-05.
Miner was greasing equipment, stepped over pipes. slipped and fell, breaking a bone in his foot and a bone in his ankle.
EMPLOYEE WAS HELPING PUT IN BELT STRUCTURE. A COWORKER HAD PUT ONE RAIL IN THE STAND. INJURED WAS TRYING TO PUT HIS IN. COWORKER THOUGHT INJURED HAD HIS IN AND HIT THE FIRST RAIL, THIS KNOCKED INJUREDS OUT, IT FELL AND SMASHED HIS FINGER
Employee was climbing onto a dozer tractor when the hand-hold he was using broke, causing him to fall back down to the ground on his already troubled left hip. An MRI revealed a fractured pelvic region. He will miss the remainder of the drying season, about mid September.
While washing SH108 with water cannon, employee fell on deck jarring shoulder. Employee sought medical attention and had surjury on 12/1/05 on his shoulder.
Employee attempted to assist in lifting a wear plate and popped something in his back/hip area.

When pouring oil from 5 gallon buckets into fct, experienced pain and discomfort to his right shoulder.The pain continued and on 9-6-05 received initial medical treatment for the alleged injury. An mri was taken indicating a torn rotor cuff. EE continued to work all scheduled days unti 1/3/06. on that date started missing scheduled work days.
Injured was pulling on a rail switch that had been run over and broken when he strained his back. Accident became lost time on 9/16/05 when it was discovered that he has a bulged disc in his back.
Fell while moving steel on the door.
He was crossing over a mobile bridge system and tripped and fell. He continued to work and started missing work on 8/30/05. The original doctor's report showed his absence due to rheumatiod arthritis but on 9/30/05 we received a MRI report showing a small rotator cuff tear in his shoulder.
EE was setting a steel roof jack under a steel I-beam at the fan housing. Picked up jack & twisted his left knee & strained his shoulder. He returned to work on 8/22/05 but was unable to return to work on 8/23/05. EE HAD ROTATOR CUFF & KNEE SURGERY.
INJURED WAS USING AN OVERHEAD HOIST AND A GRAB LIFT TO REMOVE A 2'X4'X1/2"" STEELPLATE FROM THE STORAGE RACK. HE RAISED THE PLATE APPROX 5' FROM THE FLOOR AND WAS ATTEMPTING TO ROLL THE HOIST WHEN IT REFUSED TO MOVE. HE TRIED SEVERAL TIMES TO PULL THE HOIST WHEN THE PLATE FELL OUT OF THE GRAB, STRIKING AND FRACTURING HIS LOWER RIGHT LEG
Alledged incident was to have occurred while this employee picked up a loop of a miner cable to move out of the way, he claims to have hurt his back. Cause of this injury was improper lifting. Compliance with rules/regs not a factor, protective equip. not a factor, mining equip/systems not a factor, miner attitude a factor.
Employee was cleaning belt line when his boots stuck in the mud. he slipped and fell against belt then fell to the ground hitting a piece of structure. Employee broke a rib and fractured shoulder.

Employee was eating lunch in the pump house when the pump house was struck by a truck. Employee suffered multiple injuries.
Employee was pinning shields. While pushing the relay bar out, it started to go in by itself. He tried to pull it back out to stop it when his foot slipped off the pontoon and he twisted his right knee.

Employees were pulling a 250'-14"" PVC Water pipe. They were pulling the pipe between a stationary pipe and another length of pipe using the rear bucket of a backhoe. As the pipe compressed between the stationary stand it popped out and struck the injured employee below his knee.

Employee was running a piece of equipment and asked one of the other operators to come run his piece of equipment because his back was hurting too bad. When he got up to come to work he could hardly walk. We think it is a re-occurrence and are waiting for doctor's answers.
EE taking bolt out of the belly pan on 980 f loader when the belly pan slipped and fell on his left middle finger causing the finger to split open. He had to have surgery to clean the finger and check for any other damage. No other damage was found. The dr's then sewed the finger up and sent him home with time off.
Employee was lifting a road drag on a Shuttle Car and strained his groin on his left side. This did not become a Lost Time Accident until 8/29/2005.
Employee was installing a door on a mill. He had a chain jack hooked on the mill door and to an overhead crane. Too much pressure was applied by the crane, bending the hook on the chain jack, causing it to slip off the door striking the employee on the cheek.

Employee was lifting 50lb bags of talc onto a converyor stystem when employee felt pain in groin. On September 7, 2005 employee was evaluated by doctor as having sustained an left abdominal inginial hernia. Employee continued to work until September 20 at which time employee had surgery to repair hernia. On October 3rd the company determined that the incident was work related.
Employee was stepping from a d9 dozer track to the lube truck when his right foot slipped off the edge and employee's right leg slipped between the bump and tracks.

Employee was operating miner when it skipped on slope and the cable holder pinned him against rib.
EE and another mechanic were performing repair work on a drill. He used a hammer to loosen parts on the final drive. Impact from hammer caused a splinter of metal to chip off and imbed in his left wrist area. Doctors determined that it would be best to leave splinter where it is. Missed two days work
Employee was working on repairs to loader bucket. Ratchet slipped - employee lost balance, fell backward onto ground.

The employee was assisting move a roof bolt machine and needed to roll the curtain up. When the employee was rolling the curtain up, a piece of rib fell from the intersection corner striking his left leg. The piece of rib corner was 2 ft W x 2 ft L x 2 ft H and fractured his left femur.

HE SAID HE WAS OKAY & CONTINUED WORKING. ON 8/22 HIS LEG WAS BURNING. WENT TO MED. CLINIC & DOCTOR INDICATED CAUSTIC BURNS ON LEFT LEG. MED. TREATMENT: BURN TREATMENT THERAPY FOR NEXT 7-10 DAYS.
Employee was operating rock truck co. #320 cat 777 when the top of the left front shock failed causing the truck to shake and bottom out aganist the frame. EE's left shoulder struck the side of cab. Left shock is located underneath the cab.

Employee was restocking miner cable in junction box when a slab of draw rock rolled from the rib, striking him on the right leg/foot/ankle resulting in a fracture requiring surgery 08-19-05. Employee started missing 08-18-05.

Employee was using an electric welder to stick weld stainless steel sheets to a coal feed hopper when he slipped on a sheet of stainless steel, twisted trying to catch himself and fell backwards.
Employee was helping to move some old core boxes off of the top shelf, he took a step back & lost his balance. His right foot rolled underneath his right ankle. He returned to work the following day & the client sent him home because they did not have light duty work for him. Client said that he would need a doctor's release for full duty. Drug test results: Negative.

Employee blocked and jacked the welding truck to work on it. While working under it, he called over a co-worker to assist. A third employee was also asked to help and while working on the welding truck, it swayed sideways, causing the jack and blocks to roll, placing the truck down onto its tires. The employee under the truck sustained bruises, a fractured collarbone and pelvis.
EE was carring 2-5 gal cane of iso 68 hyd. oil around a water hole. He slipped and fell one of the cans of oil hit on his little finger right hand lacerating the bottom flesh part off.

A plant technician was walking down a stair case. He was on the 4th step (from the bottom), missed the 3rd step, and fell to the floor and ruptured a quadricep tendon.

INJURED EE FELL STRIKING THE BACK OF HIS NECK ON A 6""X6"" TIMBER. HE WAS TRANSPORTED TO CHARLESTON CAMC GENERAL DIVISION. WAS REFERRED TO A SPECIALIST. ON 8/19/05 EE VISITED SPECIALIST, BUT A DIAGNOSIS COULD NOT BE GIVEN DUE TO SWELLING. MARFOLK SAFETY WAS UNABLE TO CONTACT INJ EE UNTIL 8/25/05 TO GET RESULTS.
Employee was desending a ladder when he slipped and fell. Medical treatment was sought and he had several sutures placed to close some lacerations on his wrist and elbow. he also was placed on restricted work.
Employee ws lifting a 24"" Black plastic pipe when he strained his back.
While loading cable bolts on a scoop, employee stepped onto a bundle of combination bolts & lost his footing. He fell onto his right side.
Employee was kneeling on a pivoting motor base removing tensioning nuts. When last nut was removed, base was allowed to pivot, throwing employee to deck below (7 feet 11 inches). Injury resulted in concussion, lacerated tongue, and contusions to right mid-torso.
Employee was attempting to move a piece of material(rubble stone) on a skid when an adjacent piece fell over cutting her knee. Laceration to right knee requiring stitches was result of incident.
THE EMPLOYEE WAS IN BY THE JAWS AND KNEELING DOWN. WHEN HE STOOD UP, HIS KNEE POPPED.
Employee was rolling up an air hose and he stepped on the connector causing him to twist his left foot and knee outward. Employee had pain and swelling in the left knee.
Lifting the ROPS from the piece of equipment. It sprang free, striking the mechanic and causing him to fall (93"") to the ground.
Employee was placing hook to get boulder out. Boulder moved, hook went forward then back while employee had hold of hook pulling his shoulder and arm. Employee has been treated for tingling and pain in left shoulder and arm. Employee had surgery on 3/14/06.
Employee was getting out of the 250 dump truck. He stepped down and twisted his left ankle then heard it pop.
Employee was welding on wing; received a welding flash burn to his eyes.

He was operating a haul truck, when he ran up on the berm and turned the truck over on its side.
Employee stated he was swinging a sledge hammer when he hit a piece of angle iron and it deflected off and hit him in the head causing a fracture to a facial bone. He was advised to stay off work.
EMPLOYEE TURNED AND GOT RIGHT FOOT UNDER THE SCOOP BUCKET
He strained his back when he was trying to recover shuttle car drag in cross-cut that was covered up with water pipe. Started Missing 09/07/05.
Employee was installing belt rollers on belt line when one belt roller flew back and hit him in the right side of the head.
EE WAS TIGHTENING A ROD WITH A 36"" PIPE WRENCH. HE SOMEHOW HIT THE ROTATION LEVER IETHER FELL INTO IT OR HIT IT WITH THE PIPE WRENCH CAUSING HIS HAND TO GET PINNED BETWEEN THE WRENCH AND THE DRILL HEAD MOTOER
He was changing a 25hp pump and as he pulled it over the hand rail he felt a pain in his right groin. Hernia was diagnosed on 8/30/05. Surgery is scheduled for 9/6/05.
Splitting rocks with a jackhammer. The rock split in an unusual manner and rolled pinning the operator's left leg below the knee.
Employee was working down slope. His feet slipped and he fell on his back.
EMPLOYEE WAS TIGHTENING THE BOLTS ON THE IMPACTOR WITH A WRENCH. THE WRENCH FELL, HE SPUN AROUND AND LOST HIS BALANCE AND FELL (APPROX 8 FEET) TO THE GROUND AND LANDED ON HIS FEET, FRACTURING BOTH FEET AND REQUIRING SURGERY.
Employee was in the process of changing a ramcar battery when a rock fell from between the roof bolts and struck her on the back of the neck.
Employee left truck parked in neutral without setting parking brake. Truck rolled pinching him between steel tool locker and truck.
Injured was standing four rows of bolts, back from last row of bolts when a piece of rock fell between bolts and rib striking him on head and back.
Employee bent over to set roof bolt in chuck, when a small rock fell and struck him behing the ear. He started missing 1-25-06 (due to weakness in arm) making this a lost time for August of 2005.
EMPLOYEE WAS MOVING ROCKS IN A CHUTE THAT CLOGGED AT THE TOP OF #2 RAW COAL SILO. WHILE USING A LONG PIECE OF PIPE TO SHOVE THE ROCK HE FELT A PAIN IN HIS LEFT SHOULDER. WAS LATER DIAGNOSED AS A ROTATOR CUFF TEAR AND EMPLOYEE BEGAN TO MISS WORK ON 11-22-2005.
Front wheel of rubber tired personel carrier struck coal rib causing the steering wheel to spin rapidly. The assist steering knob struck injureds left arm causing a fracture to the radius bone.
EE was hanging a check curtain when a piece of draw rock fell fromt he mine roof striking him on his legs.
While stepping down ladder off the chips stacker, foot slipped, causing strain in back.
EMPLOYEE WAS DRIVING WEDGES ON THE GRANITE TO BE LIFTED-HAMMER HIT HIS RIGHT INDEX FINGER
Employee felt pain in groin area after moving blocking for a counterweight. Was seen by physician on 08/15/05 and was diagnosed with a hernia. The incident occurred on 08/11/05, we did not receive a diagnosis until 08/15/05 and are still waiting for additional medical information as of 08/25/05.
4Injured was installing 8' combination bolts when bolt struck and injured was reaching for bolt when it struck arm (left).
Letting head down, he had his hand on drill pot. Got fingers between drill pot and drill pot arms.
Employee got on top of the engine compartment of the #30 crawler to look over the dump. He tripped on the air cleaner, causing him to grab onto the exhaust pipe, burning the palms of both hands.
Employee was using a rail to knock belt structure together. As he went on the back swing employee had his left hand down over the end of the rail and struck the tail piece.
Member was tightening up 1 7/8 inch nuts ont he storage unti when the ratchet and socket slipped injuring his left shoulder. member continued to work until sept 23, 2005.
Employee's foot slipped between push arm and track. The weight of his body pushed him to the ground with his leg between track and push arm.
Employee was climbing out of scoop bucket, operator moved bucket, catching foot in pinch point.
The ee alleges injury to his back and arms.
WHEN BARRING STONE - BAR SLIPPED HITTING EMPLOYEE'S RIGHT KNEE.
The #2 BE shovel was loading limestone boulders onto the rock trucks. The shovel operator placed a large boulder on Employee's #3025 rock truck. The boulder fell over striking the side of the truck bed causing the truck to shake suddenly. Employee was jostled causing him to strike his head.
Employee was standing on a ladder to move a hopper that was being placed on the crusher, and felt his left knee give out.
EMPLOYEE WAS OPERATING RUBBER TIRED LOADER. HE REACHED TO OPEN RIGHT SIDE DOOR AND HIT ARM REST LEVER INJURING HIS RIBS. EMPLOYEE DID NOT FEEL IT WAS SERIOUS AND CONTINUED TO WORK THRU 8/13/05. HE WENT TO EMERGENCY ROOM THE NIGHT OF 8/13/05. X-RAYS WERE INCONCLUSIVE.
Employee was changing a bogie on a D11 roller frame. Employee started to get up when the other mechanic moved the bogie, catching the employee's fingers between the bogie and the frame.
Unloading resin from scoop bucket and putting the glue on top of pinner, he felt pain in lower back, hip and left leg.
While crawling in mine, employee twisted right knee. causing swelling.
Employee was going to make an electrical exam of pump starter box at the surface pond. As he stepped over the embankment he lost his feeling and slid down the embankment and flipped over a log coming to rest on the ground. He was stating that his back and hip area were hurting. Co-worker called for emergency services and he was transported to an area hospital.
EE was climbing down into the hole of the revolving frame without the use of a ladder to weld a seam. When he got to the bottom, his (L) knee gave out on him. Reportable as of 8/25/05. EE did not seek medical treatment until 8/25/05 (1st visit to doctor): severe knee strain requiring the use of an immobilizer, crutches and sedentary work only.
WHILE REACHING BEHIND ROOF BOLTING MACHINE TO MOVE TRAILING CABLE. HIS LEFT HAND WAS PINCHED BETWEEN BOLTING MACHINE AND RIB. HE RECIEVED INJURY REQUIRING 4 STITCHES
