Small fire in the hydraulic/ MCC room at the baghouse 1-4. Fire lasted approximately 10-20 minutes.
A ROOF FALL OCCURRED IN THE 2NORTH #3 MAINS, 20 CROSSCUT, #4 ENTRY. THE FALL MEASURES APPROXIMATELY 90 FEET LONG, 18 FEET WIDE, AND 12 FEET HIGH. THE AREA IS SUPPORTED WITH 5 FOOT FULLY GROUTED RESIN BOLTS, 12 FOOT CABLE BOLTS, AND 6X6 TIMBERS.
Two employees were changing two 12 volt batteries on a welder. The employee removed the ground cable from both batteries first. The employee then removed the positive cable from one battery and were attempting to remove the positive cable from the second battery when that battery exploded.
A roof fall occurred in the NO. 2 return entry in the NO. 2 heading approximately 500' outby the face of the 001-0 section. The fall, in a slip form measured approximately 25' long x 16' - 18' wide x 0 - 5' thick. The rock fell above the anchorage zone of the roof bolts installed in the area.
Unintentional roof fall occurred in the 2nd main east #3 entry in intersection nine crosscut 85. The rock consisted of slate and layered rock measuring 30' x 18' x 9'. The date of development is 7/10/93. 6' and 8' TR bolts were the support method in the fall area.
MINE FOREMAN AND MSHA ROOF CONTROL INSPECTOR WERE TRAVELING INTAKE AIRWAY #7 ENTRY SPAD #169. DISCOVERED ROOF FALL MEASURING 20 FT WIDE X 20 FT LONG X 5 FT THICK AT 10:00 AM. FALL WILL BE TIMBERED OFF AND ESCAPEWAY REROUTED. THIS AREA WAS MINED SEPTEMBER 2004 AND BOLTED WITH 4 FT RESIN BOLTS. THIS AREA WAS LAST FIREBOSSED ON 9/22/05.
A roof fall occurred on the 1C panel at crosscut 15 in entry #6. The cause of the fall was weak laminated shale. The fall was in the entry adjacent to the primary escapeway for MMU-001
MINE FOREMAN WAS MAKING FIREBOSS RUN AT APPROXIMATELY 4:30 AM DISCOVERED ROOF FALL IN #3 ENTRY MEASURING 10 FT WIDE X 20 FT LONG X 5 FT THICK. 40 FT INBY SPAD 160. THIS AREA WAS BOLTED WITH 4 FT RESIN BOLTS. FALL WILL BE CLEANED UP AND REBOLTED WITH 6 FT BOLTS AND ANY ADDITIONAL SUPPORT.
While starting a brush fire the operator attempted to egress the area. He was clearing the roadway in front of the excavator; his left track trammed upon a stump which caused the excavator to overturn. The heat and blowing embers caused the excavator to catch fire. The operator was wearing his seat belt and received no injuries. The excavator was a total loss.
FALL ON #1 UNIT BELTLINE, X-CUT 46, 3 X-CUT OUTBY TAILPIECE #4 ENTRY. LENGTH 20', WIDTH 20', THICKNESS 4-5'.
A fall of the roof occurred on the 5th s.w. mains, 368 belt conveyor at x-cut 95. The fall was 50' x 20' x 7'. no personal or ventilation were affected. the fall occurred int he bolt entry.
During #1 skips dumping cycle a rock lodged in one gate latch preventing it from closing properly. When the skip was loaded one side of the gate opened partially jamming against the shaft timber. The skip had to be emptied by hand to free and latch the gate, no injuries or damage resulted from the incident.
Garden B-shaft hoist was inoperable due to elevator grating damaged while loading scoop onto elevator.

An unintentional roof fall occurred at the Galatia North Portal at Crosscut 31, Entry #3. The fall was approximately 10' long, approximately 10' wide and approximately 4' thick. The fall did not affect ventilation. The fall will not be cleaned up.
A fall at the anchorage point occurred on the longwall tailgate resulting in loss of egress for approximately 15 minutes. The fall measured approximately 16' x 16' x 9'. It was located 35' outby interesction from survey tag station 11890.
A roof fall approx. 50' long x 20' wide and 1' to 4' high occurred in #6 entry of 1 left. The fall material was slate with a slip at the top. There were no injuries and no property damage
A roof fall was discovered by the examiner while making his weekly examination of the 1st West Panel return air course. The fall was located in the No. 3 entry and originated in an intersection 1 xc outby spad 32 + 99 (xc44). The fall measured approximately 20' x 45' x 5' high.
The fall was int he #2 (N. intake) entry, which blocked escapeway from the #1 unit no work will be done on #1 unit until escapeway is sate to travel. The fall was 70' long, 5' high, 18' wide. The fall will be cleaned up before work is rejumed on the affected #1 unit.

A ROOF FALL WAS FOUND ON 9/26/05 AT 9PM BY EXAMINER DOING PRESHIFT. FALL WAS AT INTERSECTION OF #26 SEAL 1 WEST. FALL MEASURED 20X20X5' HIGH. AREA WAS MINED 6/15/83 AND HAD BEEN SUPPORTED WITH 5' FULL RESIN BOLTS. AREA IS BEING PROPPED OFF & 6 NEW SEALS WILL BE BUILT TO AVOID THE AREA. NO INJURY OR PR0PERTY DAMAGE RESULTED.
An unintentional roof fall occurred at the Millennium Portal at Crosscut 36, Entry #1, 7North longwall tailgate / 6North headgate. The fall was of unknown length, approximately 18' wide and approximately 15' thick. The fall did not affect ventilation. The fall will not be cleaned up.
500KW generator portion of MG set was showing temperature readings in excess of nominal. Investigation revealed some broken springs used to apply proper brush friction. All brushes and some springs with holders were replaced. Topeka office was notified of hoist shutdown and re-start.

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE GALATIA NORTH PORTAL AT CROSSCUT 14 IN THE FLANNIGAN UNIT, CROSSCUT LEFT. THE FALL WAS APPROX 20'L X 15'W X 7-8'THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF MEN. THE FALL WILL NOT BE CLEANED UP.
Unintentional roof fall above anchorage point fell in crosscut at Survey Station 971 in Return air course. Fall occurred sometime between weekly exam of return and was discovered during weekly exam on 9/22/05. No injury - no equipment involved.
A Komatsu dozer 375A was pushing coal in the clean coal stock pile at the plant, and the operater noticed that smoke was coming from the back of the dozer. We could not extinguish the fire in 30 minutes, but it was contained.
Roof fall occured in the intake intersection of New 2nd Right Panel, #6 entry at x-cut #9. 800' outby the face. The fall measrued 20' x 20' x 7' thick. The area was known to be bad and had supplemental support and cribs all along the area for 3 x-cuts inby.
THE ELEVATOR IN #6 AIRSHAFT WAS INOPERABLE FROM 5:30AM UNTIL 8:00AM DUE TO BUILD UP OF MOISTURE IN THE CONTROL BOARD. ONE EE WAS ENTRAPPED UNTIL 6:30AM.
A roof fall measuring 30'L x 20'W x 6'H occurred 1400' outby 001 section in the no. 1 entry. Location spad 5667. Area was bolted with 5' bolts.
HOISTING INCIDENT : COWLING PORTAL ELEVATOR - On 09/21/2005 at approximately 04:10 pm, the Cowling Portal Elevator experienced a braking system malfunction which interfered with the use of the hoist for approximately 70 minutes. The elevator was back up and the brakes were functioning properly at approximately 05:00 pm.
THE ELEVATOR IN #6 AIRSHAFT WAS INOPERABLE FROM 5:00PM UNTIL 6:15PM DUE TO A MALFUNCTION ON A PRINTED CIRCUIT CARD IN THE CONTROL BOARD. ONE EE WAS ENTRAPPED UNTIL 5:50PM.
North 6, 16 block, 6 entry, MMU 004, 4-way. 17 ft wide x 20 ft long x 7 ft high. Support: 6 ft 3/4 inch resin bolts, roof mats, wire screen, cable truss bolts. Fall dangered off, additional roof support installed around fall. Fall will be cleaned up. Clean-up plan posted.
An unintentional roof fall occurred on the South East Main return, entry #6 and the intersection of 25+90 x-cut. The cause of the roof fall was a slip and unconsolidated shale above the anchorage point. The area had 6' fully grouted rebar roof bolts and wire roof matting installed for roof support.

Unintentional roof fall inby Section dumping point, between Survey Station 1191 and 1230, no injury, no equipment involved, fall investigated by MSHA, not cleaned up.
A roof fall was discovered in our slope on 9/18/05 at 3:30 pm due to power shortage for the surface area. MSHA was notified this day and was onsite 9/19/05. The mine was not in operation this day and nobody was underground. A contractor is performing the cleanup and resupport to the roof and we plan to be in operation by 9/24/05. The main power cable is being repaired also.
Size of fall: length 20ft. width 30ft. height 4ft. Roof support: resin. Number of cribs /timbers installed prior to fall - none. Type of strata: shale/laminated. No coal rider seam present. All entrances are breakered off, area of roof fall will be rehabilitated. Distance from active face: 2300ft. No personal injuries involved.
A reportable roof fall occurred in 19 Tailgate, entry 2, crosscut 41. The fall was approximately 18 feet wide, 18 feet in length and 9 feet high.

A roof fall occurred 1 break inby spad #1896 in the #4 entry. Fall measured 60' long, 18' wide, 0-8 foot thick.

AT APPROXIMATELY 10:30 AM ON SEPTEMBER 17, 2005 A ROOF FALL WAS DISCOVERED AT THE 3 + 00 SURVEY STATION OF THE MAINLINE JUST INBY THE BENTWOOD PORTAL DRIFT OPENING. THE FALL MEASURED APPROX. 17' WIDE X 40' LONG X 15' TO 20' HIGH. THERE WERE NO INJURIES REPORTED AS A RESULT OF THE FALL.
Unintentional roof fall occurred on the 001 working section, in the #6 entry at #8. This area was supported with 10' point anchor roof bolts. Supplementary roof supports were installed around the roof fall area and the fall will not be cleaned up.

As a skip was being brought to the surface to be dumped, the hoist experienced an underwind fault. At which time the brake system set as it should. The brake system was unable to hold the loaded skip, which went back down the shaft resulting in damage to the head frame structure, structure in the shaft, ropes, and the hoisting building. There were no injuries as a result of this
A roof fall occurred on the 4 West B at crosscut 114 between entry 4 and 5. The cause of the fall was weak laminated shale. This area was already cribbed next to the road and timbered next to the belt. The fall was in the area between the road and belt.
The auxiliary mini cage located at the production shaft was removed from service due to damage to the head frame structure, that resulted from a loaded skip going back down the shaft. There were no injuries as result of this incident.
An unintentional fell of roof occurred on the continous miner that was approx 20' x 20' x 20' x 6'. The fall occurred on the his left panel off wild cut submains two breaks inby spa d#3398.
A fall occurred in the #7 entry at spad 200 measuring 19' wide x 20' long x 5' high. The fall will be timbered off and dangered off and will not be cleaned up. This fall is 1800' from working face.
There was no evidence of unsafe conditions. Discovery of the fall was spet 15. about 2 pm. The fall was discovered as a result of checking on discharge water pipe. The fall had broken this pipe. Review of fan chart recorder data would indicate this fall occurred around 6 am. There was an increase in mine air resistance from 1.2 to 1.4 h2o gauge.

At the east grizzley heat was generated from warm coal in old duct work left from a de-commissioned bag house. To allow water to drain, a 6?? hole was cut into a cover plate where a pipe was once connected to the baghouse . As the water began to flow from the hole, dust was carried with the water and flashed. The flash occurred outside of the facility.
A cam lever bracket broke off of its mount at the bottom of the elevator shaft. This would not allow the elevator to function properly. It took 44 minutes to repair the bracket.
on 9/13/05, an unintentional roof fall occurred along the 2nd west beltline (#5 entry) at #75 cross cut. This area was supported with 10"" point anchor roof bolts and the roof fall will be cleaned up and re-supported.
Employee of Mine Resource dust samples car caught on fire and burnt. No injuries.
The #8 elevator was out of sevice for a period excedding 30 minutes when the service personnel from Schindler took the elevator past the top landing and the over travel stopped it. The rope brake set at that time. The elevator had to be backed out of the over travel and checked for damage and returned to service.

Confractor alimark crew had the raise climber get stuck in the raise about 100' up when a chain used to secured stopper got caught in the sprocket track. the extra car was installed on the track and they came down after about 1 1/2 hrs.

The slope hoist at our Wheatland facility malfunctioned. The cause of the malfunction was the upper travel limit switch. The switch was cleaned and the hoist returned to service.

A roof fall occurred in the 2 Main North, #7 entry, intersection of crosscut #2. The fall measured approximately 30'X 19'X 6'. The area was bolted with 6' tension rebar bolts. The area was mined in November of 2000. An undetected slip was determined to be the cause of the fall.

A roof fall occured in the East Mains, #1 entry, in the pillar line between crosscut #31 and #32. The fall measured approximately 40'X18'X6'. The area was bolted with 6' tension rebar bolts. The area was mined in October of 2000. An undetected slip was determined to be the cause of the fall.
A roof fall above the anchorage of the installed roof bolts was found mine the mine examiner. The fall was approximately 20 ft wide by 20 ft in lenghth by 6 ft in height. The area of the fall was in the intake airway but did not effect the escapeway.
An unintentional rock fall was discovered in the #1 entry beginning at spad #2129. The area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will be cleaned up.

When mill started up, power was lost. Power was restored at 7:15 a.m., hoist was tested and miners returned underground at 7:25 a.m.

The #8 elevator was out of service for a period exceeding thirty minutes due to the malfunction of the counterweight buffer switch. The switch was changed and the elevator was resotred to proper operation.

The fall occured due to the fact of roof failure at and just above the anchor point. The fall measured 20' by 17' by 5'. On 9/6/05 an unintentional roof fall occurred on the 1st 42 South belt line, in #5 entry #45 cross cut. This roof fall was located in an intersection and was supported with 10 foot point anchor roof bolts. The roof fall measured 20' in length by 20' in width and 10' in thickness. The roof fall was cleaned up and the area was re-supported.

GARDEN B-SHAFT HOIST WAS INOPERABLE DUE TO AN ELECTRICAL FAULT IN THE GATE SWITCH.
While excavator operator was mining in open pit south curtis wright area he noticed diatomaceous ore movement from his right side he quickly tried to back up machine and get out of way. Classic wedge failure in wall occurred which caused the safety catch bench above excavtor and highwall above it to fail brining down debris and partially covering/trapping excavator.
Slope brake car derailed. Brake car was out of service for 2 hours and 2 minutes.
Had roof fall on the 2nd M. North belt line, at +4681, intersection #4 entry. Belt line was timbered up to this area, approaches are timbered. Fall will be cleaned up. Belt, structure and water line, under fall. The fall is appx 70' long, 4 to 6' high, 18' wide, had been supported with 5' tension roof bolts.
The mines hoisting system became inoperable for a period of time in excess of 30 minutes when a mine site electrical power outage occurred. The failure occurred when an automatic reclosure switch failed. Additionally, during the outage all underground personnel were brought to surface.

An unplanned roof fall occurred int he #5 entry about 2 breaks inby spad #122998. The fall measured 30' x 20' x 7'. All entrances to the fall were breakered off and area will be rehabilited. There were no injuries reported but passage of person was affected.

An ignition occurred at the face of #11 entry, right 3 north section at 327+59. There was no damage to equipment and no injuries.
Employee was driving a Superior Bowen's (Contractor) water truck down the slope into mine and lost control, impacting a dome out (located at 2N- 0 E/W) at the bottom of slope resulting in ramping up of truck over and through to other side.
(Under investigation) (Per Supervisor) Employee was assigned task of pushing overburden wall down with bulldozer. This was being done to install a water jack (floating pump) to pump out old mine cut. A short period of time later bulldozer and employee were discovered under water in mine cut by another employee.
Employee fell to concrete floor at base of preheater - apparently from the second level where he had been performing work. Death was apparently immediate. There were no witnesses to the accident and it is not known how the employee fell or whether he tried to access his work from the safety handrail along the catwalk.
Employee was operating a 50 ton Cat end dump truck & was at dump site with his 3rd load of stone from pit. While attempting to dump the load from truck at dump site, which is elevated some 20' above the plant, his truck went over the dump face backwards & flipped end for end 180' landing on its top, wheels to sky, at bottom of dump site at plant level. He was still in cab.
Victim was operating a motorized scraper, without a seat belt. The belt was in good condition but he was not using it. He ran over a large pile of dirt and fell out of the machine. The machine ran him over.
Employee was working on new fletcher bolter drilling a gas hole. Auger got stuck so they dropped rotation motor & tried to remove auger from chuck. Didn't work so they raised auger back up & clamped centeralizer & lowered feed motor. Auger came loose from centeralizers & struck employee on right ring finger.
Ee was carrying a acetylene bottle from last open break toward tool cars. He tripped over half header and fell face first toward the mine floor. The actylene bottle fell against ee rt. hand.His rt middle finger was cut off at 1st joint and his rt. ring finger was cut and mashed. Finger was put in ice and sent to hospital. Unable to reattach finger.

A metal box shute was being cut loose when the final cut was made the box moved forward catching Ee's two middle fingers ont he left hand at the nail mashing off the tips of the two middle fingers.
Employee was cleaning a rare earth magnet used to capture nuts and bolts from a product stream. While the employee was handling the magnet, it was magnetically attracted to its mount which caught two fingers of his left hand between the magnet and its mount, causing a fracture and laceration to his pinkie finger and amputation of the tip of his left ring finger.
Employee was rolling 100 lbs (plus) rocks away from equipment.
Employee was putting a liner in the mill. A pin that holds the liner broke and liner fell on his left hand's ring finger. Weight of liner mashed his finger.
Employee felt an abdominal pain coming on during his work week.
EE STATED THAT THE FEEDER SHEARED A PIN FOR THE CONVEYOR. EE STATED THAT HE ATTEMPTED TO CHANGE THE PIN WHILE THE FEEDER WAS RUNNING AND HIS RIGHT INDEX FINGER WAS AMPUTATED AT MIDPOINT OF THE NAIL. HE WAS TAKEN TO HARLAN HOSPITAL FOR TREATMENT. THE DR RELEASED HIM FOR LIGHT DUTY.
Employee had installed the engine and hydraulic pump assembly in the 463 Bobcat and was lifting the assembly with a pry bar to slip the engine mounts into place. His fingers were under the mount when the pry bar slipped, dropping the assembly on his finger, amputating the last joint on his left index finger.
EE was operating a loading machine in 28-B North in #3 entry. He had just finished loading a shuttle car and when he looked back, he saw that the shuttle car had caught the loader cable and was dragging it around the corner. He got out of the shuttle car as the buggy stopped and as he straddled the loader cable the buggy started back up, pinning his left thigh against the loader.
Got out of vehicle, stepped on rock, twisting ankle and fell on right shoulder

Was operating forlift truck & was climbing on & off to help guide the 350 MCM electric cable that powers the dyna-jet. It started hurting early in the afternoon but at around 5:45 it started hurting so bad that I could not finish out my shift.

An employee on a forklift set a loaded pallet down on the toes of a fellow employee, causing trauma to foot. There were no broken bone according to x-rays and he returned to work after missing only one day of work.

Employee was standing beside a concrete pump pipe. He removed his safety glasses to clean them. Some concrete hit him in the face thru a leaky joint. He received a bruise, and complained of eye irritation. Eye was cleaned and ee went to dr. Dr gave him 3 days off. No serious injury suspected.
Sometime during his workshift on 9/28 EE got sweat & foreign objects in eyes. Wind blowing 30 + mph. When woke on 9/29 eyes were irritated. Went to clinic ...no objects found in eyes but antibiotic prescribed.
EMPLOYEE REPORTED: HE & COWORKERS WERE HANGING HIGH VOLTAGE CABLE, FELT PAIN & SORENESS TO HIS SHOULDER AFTER COMPLETING THE JOB. IN 1999 HE TORE THIS SAME ROTOR CUFF, REQUIRING SURGERY. IN 2002, THIS EE EXPERIENCED ANOTHER TEAR & SURGERY WAS REQUIRED AGAIN. THE MINE WAS NOTIFIED 10/21/05 THAT EE HAS AGAIN TORN THE SAME ROTOR CUFF & SURGERY IS NEEDED TO REPAIR THE TEAR.

Employee was walking along panline on headgate side of shearer between #174-#175 shield F-11 longwall face. He had his right hand on the handrail and a piece of rock, 6"" x 10"" x 18"" fell from between the shields striking him on the right hand, causing a fracture to the right little finger. Employee began missing work on 10-05-05, and returned to work on 10-07-05.
Employee was removing a radiator cap from dozer, his face was burned by escaping steam.
Employee was returning from lunch and stepping up onto the excavator when his right knee locked.
Employee began missing work on 9/29 from an injury that occurred on 8/25/05. He was using a bar when the weight of the part being moved shifted and he felt pain in lower ribs and lower back.

While getting on top of the sheares to put hydraulic oil in, he grabbed the spray bar with his right arm to help pull himself up and felt pain in his right shoulder. He continued to perform his regular duties. He was later daignosed with a right rotator cuff tear.

Employee began missing work on 9/28/05 from an injury that occurred on 8/15/05. He was walking to supply area when he bumped his head into the mine roof. He then twisted his lower back.

Injured was moving a ABS 8 hp pump in the #3 entry. While moving pump injured slipped straining his lower back.
Employee had just put a pin in the ripper and as he was standing up the ripper shifted causing the chain from the boom to drop hitting him on the shoulder.
Employee ran over a rock while driving a Mac 3 causing the steering wheel to spin fracturing his finger.

EE was installing a wipe ont he belt press. He was standing on a conveyor in front of press. He pulled on a rope to move lever. The rope came untired and he fell to the slab below 6' He did not wear fall protection.
EE was attempting to pry rock off of the top of a continuous miner with a slate bar. When the rock slipped causing continuous miner with a slate bar. When the rock slipped causing the other end of the bar to come up into his midchest area. He was admitted overnight at Raleight General Hospital for observation.
Employee was putting oil in shuttle car when a piece of rib rock broke from between pins and struck his lower leg.
Employee was backing a loaded Cat.777 hauler when he backed onto a berm and the truck turned over into a dry pond.
Employee was pulling conduit to re-connect the compressors. Musculskeletal pain.
EE WAS ON BARGE SWITCHING OUT, WHILE LIFTING WIRE LINE EE STRAINED MUSCLE IN HIS BACK.
Ee was attempting to adjust height on back left stud because of change in depth of water. His foot slipped off of step and he fell approx. two feet. Resulting injuries were a bruised knee and pulled leg muscle. EE did not report the injury until six days later. He has a dr appt. today (Monday, Oct-3) He has missed no time as a result of the injury at this time.
EMPLOYEE WAS CHANGING ROLLERS ON THE 1ST MAIN NORTH. AS HE WAS MOVING TO THE NEXT LOCATION, HE STEPPED ON A LOOSE STONE TWISTING HIS RIGHT ANKLE.
EE WAS WAITING ON HELPER TO SPOT A BOLT BEFORE ENTERING #2 LEFT TO BOLT WHEN A 12"" BY 12"" PIECE OF ROCK FELL BETWEEN BOLTS STRIKING OPERATOR ON THE HARD HAT BOUNCING OFF OF HAT STRIKING HIM IN THE RIGHT FOREARM CAUSING A LACERATION ABOUT 4"" LONG ON ARM. RECEIVED 17 STITCHES.
Employee was lifting a wood block when he pulled a muscle. The result of this was improper lifting techniques.
EE #1 was loading a RR car underneath the loading glove. EE #2 was above the loading chute preparing to bring more empty cars from the storage area. While letting the air off the first empty RR cars, they took off down the track. These runaway cars struck the car being loaded under the chute knocking EE #1 off of the car. The collision resulted in fracture of his right foot.
EMPLOYEE STATED THAT HE WAS OPERATING PARTS JEEP ON L-10 TRACK DELIVERING PARTS TO THE LONGWALL. AS HE WAS DRIVING THROUGH THE INBY DROP CURTAIN, HIS ARM GOT CAUGHT IN THE CURTAIN. HE WAS DIAGNOSED WITH A ROTATOR CUFF TEAR TO HIS RIGHT SHOULDER. HIS FLWD DID NOT OCCUR UNTIL 11/17/05.
Employee was walking in the shop building in the area where there is a floor level difference of 7 1/2 inches. He was crossing the ramp between the two levels when he stepped on the edge of the ramp. He has a torn Achilles Tendon, a torn Calf Muscle and torn Ligaments in the Knee area of his left leg.
Unloading equipment and flag pole came out. Employee did not notice and jabbed flagpole in eye.

Employee was working at the 18 west longwall belt storage unit. He was lifting and pulling belt fabric over a belt winder. He experienced a burning and stinging sensation in his left groin area. A lump appeared in his left groin.
EE WAS ON ELEVATOR. WHEN CLOSING THE DOOR HIS RIGHT HAND BECAME CAUGHT BETWEEN THE DOORS SMASHING HIS MIDDLE AND PINKY FINGERS.
EE WAS TRAMMING A 16G GRADER TOWARD THE PARKING LOT WHEN THE MEMBER RAN INTO A SUMP AND EJECTED FROM THE GRADER
EE was using a steam cleaner/power washer. He was cleaning off some equipment for repair. He had his hand and finger on the wand and trugger. As he pulled on the hose, the wand pointed downward and hot water spray struck him on the top of the foot.
While helping the welders remove guard on the driver fan, EE removed the bolt and kicked the guard with his left foot to loosen it. As a result his big toenail hooked onto the safety shield guard in his rubber boot and ripped the toenail back.
Injured stated he was drilling a hole in the mine roof when he hit a crack and let his drill steel come out of the chuck stirking him on the fingers.

EE WAS HOSPITALIZED 9/18/05 WITH INFECTION IN BOTH HANDS. 9/19/05 EE CALLED IN TO REPORT HE WOULD NOT BE AT WORK WHEN QUESTIONED ABOUT INFECTION HE SAID HE DIDN'T KNOW BUT THAT HE HAD NOTICED A PUSSY SORE ABOUT 2 WKS PRIOR TO GOING TO HOSP. 9/23/05 HOSP REQUESTED WC INFO AND AT THAT TIME EE STATED WEARING OLD GLOVES CAUSED INFECTION.
While performing his normal duties as a scoop operator employee experienced pain in right knee. Employee came back to work on his next scheduled shift and continued working until October 10, 2005 when he started missing work due to this injury.
EMPLOYEE WAS PULLING AN IRRIGATION DISCHARGE HOSE AND STEPPED BACK INTO A TIRE RUT AND TWISTED HIS RIGHT KNEE. EMPLOYEE DID NOT GO TO THE DOCTOR AND INJURY DID NOT BECOME REPORTABLE UNTIL 10/27/05. REASON FOR REPORTING: RESTRICTED DUTY.

Employee was on the radio stacker belt when the plant was turned on. He was shaken off and fell from the stacker belt. Employee sustained a broken femur.
EE WAS OPERATING THE ROOF BOLTER WHEN HE HAD PLACED HIS LEFT HAND UPON THE CANOPY OF THE ATRS. HIS FINGER WAS PINCHED BETWEEN THE MINE ROOF AND THE ATRS. HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE RECEIVED 3 STITCHES. THE HAND WAS X-RAYED AND SHOWED NO FRACTURES.
Employee was on knees mixing B-Bond. Safety glasses were fogged up. He removed safety glasses to let dry & continued mixing up B-Bond and the B-Bond splashed into his face and into right eye. He went to boss and boss flushed out eye with water. Employee finished shift and completed job of building a stopping. Next day eye was swelled shut. EE went to Dr.
Employee had accumulations of rock in knee pads. On attempting to kneel down he states sharp pain felt in right knee. Area progressively became swollen, warm to touch over the following week. Sought treatment on 9-27-05.
Employee was walking belt when a rock fell, knocking timber over on right ankle. This knocked employee into belt. The belt rubbed his left arm & left thumb and the belt rolled him, causing him to twist his back.

While in the process of closing a valve on Pipe #2 at the CIC, the employee felt his right knee buckle to the inside of his stance and a sharp pain in his knee.
on 9-22-05. Ee stated that he slipped and fell on a mat uner a mine phone and landed on his backside, He returned to work on his next shift after being released from physician to do so. On 10-7-05, he ceased coming to work and did not Mgt aware the he was seeking another dr opinion. The second dr removal him from work and place dhim on the port, mgt was not make aware of 2nd dr.
While employee was burning the shaft on the no. 2 bridge a hot spark entered left ear. Employee continued working until January 25, 2006 at which time this became a reportable as he started missing work in order to have surgery done to graft skin at burn location.
THE BELT ON THE CRUSHER HAD STOPPED WOTKING WITHOUT ANYONE'S KNOWLEDGE. WITHOUT INSTRUCTION, INJURED BEGAN TO APPLY BELT DRESSING TO THE HEAD PULLEY OF THE CONVEYOR BELT FEEDING THE SCREEN AND CRUSHER. HIS ARM SOME HOW GOT CAUGHT BETWEEN THE BELT AND ROLLER
Employee was going outside to get supplies. He was pulling mantrip into crossuct and front tire hit the rib spinning the steering wheel. Steering ball on the wheel went between the middle and index fingers.
Driver was in the process of having the tank trailer loaded by a specialty minerals ee. When complete, the rebar driver started to walk back to the tractor when his hard hat and then shoulder was struck by a piece of metal.
While inspecting and verifying fluids of assigned equipment, he twisted his ankle.
Roof Bolter was drilling hole in roof when drill steel hung in top, roof bolter operator was trying to pull steel out of hole when he push wrong lever on roof bolter mashing left hand.

EE STATED THAT CRAWLING IN LOW/WET CONDITIONS WAS CAUSING HIS KNEE TO HURT
The injured was operating a remote controlled continuous mining machine while mining a lift right out of the No. 1 entry. A piece of rock rolled from the rib and struck the injured, resulting in a fracture to his right ankle.
Injured was riding in the rear facing seat of a diesel mantrip. Mantrip went around a switch and derailed. When the mantrip derailed and struck the rib, the injured's momentum drove his left kidney area into the SCSR. Injured suffered a briused kidney, ribs and back, Mantrip was being operated at an excessive speed.
While running a push-pull scraper operation in pit, the operator of the pull scraper pulled up a rock that the push scraper hit, causing the operator of the push scraper to be bounced, hitting his head on the roof of the machine.
EE WALKING IN FACE OF #5 ENTRY. WEST MAINS WHEN HE SLIPPED ON WET MUDDY SIDE OF PITCH AND FELL ON SELF RESCUER.
Employee stated that while he and 3 other employees were lifting a pan pusher onto the shearer he felt a sharp pain in his groin on the left side.
Employee was on his knees shoveling out the feet when screen wire being transported on the panline hit a rock causing the wire to strike employee on the left shoulder. Lost time injury.
While approaching the H-3 underpass from Phase II in truck #1389, EE locked on his breaks when he noticed our service truck standing by to let him pass. When employee locked on his brakes it caused the truck to bounce up and down, thus aggravating his back.
Build up from the preheater dropped onto the feed shelf, which caused hot kiln dust to spill from the feed end air seal. This spillage then fell 26 feet to the ground floor.
EMPLOYEE GOT OFF RAMCAR AND TWISTED HIS LEFT KNEE - WALKING. Employee did not start losing time until 1/19/06 due to surgery.

WHEN THE EMPLOYEE PLACED THE LADDER UNDER THE CRUSHER PLATFORM, HE DID NOT PROPERLY STABILIZE THE BASE TO PREVENT MOVEMENT. THE ANGLE OF THE LADDER CONTRIBUTED AS WELL BY PLACING EXCESSIVE BURDEN ON THE BLOCKS THE EMPLOYEE USED.
The employee attempted to stop a stack of roof mesh from tipping over. He tripped getting out of the way and the mesh struck him as he was on the mine floor.
EE WAS WALKING UP #4 ENTRY WHEN HE WALKED INTO A ROOF BOLT THAT WAS HANGING DOWN FROM THE ROOF. HE WAS TAKEN TO THE HOSPITAL WHERE HE RECEIVED STITCHES TO THE RIGHT EAR.
Employee was stripping copper blanks at the 2 stripping machine when he felt a pain in his lower back. Employee then fell back onto the floor grating.
Employing was lowering light tower, cable had slack, slipped and cut end off of employee's thumb.
JEEP COMING OUT OF A PLACE AND ANOTHER JEEP CAME ACROSS IN FRONT OF IT. THE JEEP THAT CAME ACROSS IN FRONT OF THE OTHER JEEP WAS THE MAN THAT WAS HURT
Employee was performing cleaning duties at Cement Mills area. Suddenly cement dust fell into his left eye causing him an irritation. He was wearing safety glasses.
Employee was using an impact gun with a wobble head socket. The socket broke and a piece of metal from the socket cut the palm of his left hand where the thumb joins the hand.
EMPLOYEE STATES WHILE TURNING ON WASHDOWN HOSE, END OF HOSE WHIPPED AROUND, STRUCK EMPLOYEE IN FACE, SPRAYING EYE.
The injured is a roof bolter, he said he had a bolt stuck in the hole in roof when he pulled the bolt out of roof he mashed his litte finger between the bolt head & the wrench on bolter.
EE was between two pieces of parked idle equipment trobuel shooting a amintenance problem. One unit rolled back pinching injured leg between the two.
EE was shoveling coal onto the tailpiece when he felt pain in his lower back. EE continued to work the rest of the shift and consulted a dr after he got off work. He was x rayed and determined to have a muscle strain.
The employee was loading 16 foot bacon strips onto the bolter when a partial bundle of 9 foot mats fell off of the cab of the bolter. The mats struck him in the back and pinned him in the bolter's walkway.
Employee was standing on the hose chain of the Highwall Miner and as he was stepping off onto a support beam he slipped and fell a distance of approximately three (3) feet.

Injured worker had climber the ladder to clean a grizzly screen. As he stepped onto the platform, he struck his right knee against the guard rail. He continued to work his shift. His knee was swollen the next morning. He sought medical attention on 9/20.

Injured man was getting in his personal car to go home. Coal truck passing by on haul road from mine to plant let his left wheels get off the edge of road, causing truck to turn over on its side. Coal came out of the truck bed, striking injured man's car. The car moved striking the injured man's leg.
OPERATOR OF TRUCK #337 WAS FUELING HAUL TRUCK. SHE STEPPED FROM TRUCK CATWALK TO FUEL TANK CONTAINMENT WALL AND TWISTED HER RIGHT KNEE.
After installing a curtain board for ventilation controls, a piece of draw rock fell between the permanent roof supports roof bolts. Striking EE on the shoulder and lower back. He missed work on the 10/10/05 threw 10/13/05.
Employee lifted too much weight at one time causing a hernia in right groin. Employee notified his supervisor on 10/1/05 of the injury and that he would be going to the doctor. Paperwork not received in company safety office until 10/10/05. Steve Pilling at Green River, WY MSHA office notified of incident circumstances on 10/11/05.
Miner operator bounced up and hit top #4 heading; hit his head and said his back was hurt.

AN OXYGEN CYLINDER TRAVELED THRU THE PICK BREAKER PORTION OF THE STAMLER FEEDER/BREAKER. A PICK CAME INTO CONTACT WITH THE CYLINDER WHICH RUPTURED THE CYLINDER, CAUSING AN EXPLOSION. THE EMPLOYEE RECEIVED 1ST AND 2ND DEGREE BURNS TO HIS ARMS, HANDS AND FACE. FIRST LOST WORK DAY OCCURED 9/20/2005.
While hanging a strap attached to the dragline powercord to a hook on the shoe to help move the cord, the injured got his finger caught between the strap and the shoe causing a broken finger.
Miner was walking on the level past the powder storage magazine and stepped on a rock which rolled and he twisted his ankle. X-Rays showed no broken bones just sprained right ankle.
While employee was climbing down a fixed ladder his foot slipped off the rung and he caught himself with one hand, causing pain in his left shoulder. He did not want to go to the doctor until 10/10/05 at which time the doctor put him on modified duty. EMPLOYEE STARTED LOSING TIME ON 2/21/06. MRI SHOWED TORN ROTATOR CUFF - SURGICALLY REPAIRED ON 2/21/06.

While employee was repairing the grizzly car at the feeder the backhoe operator went to set the grizzly bar and when it was lifted the grizzly bar swung hitting employee causing him to fall approx. 13 feet to the ground onto # 1 belt. (9/22/05 call to company - possible paralysis)
7H section using a sledge hammer to remove belt structure, he accidently hit the belt, causing the hammer to deflect and strike his right shin. causing abrasion which became infected.
While attempting to install 6' resin roof bolt, employee pushed roof bolt into hole with drill pot. The roof bolt bent and flew back striking his left wrist. The accident occurred on west 1 section. FLDW occurred on 9/20/05.

Putting up outside roof bolt when a piece of rock fell hitting victim on left hand ring finger, cutting finger.
A 488 scoop ran over a 6' timber. One end of the timber rose into the air and when it returned to the ground it struck the employee on top of his left foot causing a possible hairline fracture.
EE STATED THAT HE WAS WALKING ACROSS THE SECTION WHEN HE TRIPPED ON THE MINER CABLE. HE FELL ON HIS SHOULDER AND WAS TREATED AT THE HARBOR HOSPITAL. X-RAYS SHOWED NO FRACTURES, ONLY BRUISED. HE WAS OFF WORK ONE SHIFT AND RETURNED TO WORK FULL DUTY ON 9-19-05.

Lost Time Accident: ME #4 Belt - Draw rock fell out of head hole while beltmen were setting a drive. Employee got on top of belt to examine the roof. While in the process of getting down, stepped off belt into scoop bucket. Then, when getting down off scoop, stepped on a rock (baseball size), twisting left ankle and falling to the ground.
A rock came over the headgate arm of shearer and handrail, pinning employee's foot between the rock and the shield.
He was handling structure in the process of pulling bed rails out of a belt box. He felt pain in the right side of his lower back.
WHILE EE WAS ATTEMPTING TO CLEAN THE SUCTION ON THE RIGHT SIDE BOLTER CHUCK OF THE CONTINUOUS MINER IN F-TAILGATE, HE WAS STRUCK IN THE RIGHT HAND BY A DRILL STEEL THAT WAS PREVIOUSLY STUCK IN THE ROOF. DIAGNOSED AS A FRACTURED RIGHT MIDDLE FINGER.
Employee got under shaker, removed guard from tail pulley and placed it leaning on shaker frame. He turned around and the vibration of shaker made guard fall on his back. On 8-26-05 employee was told not to be around shaker on tail pulley when it was running. This was the reason he was starting work at 5:30 a.m.
As employee exited the stone pit in a loaded R35 Euclid rock truck via the haul road ramp the R35 rock truck stalled and coasted backwards down the ramp. The load shifted causing the R35 to roll onto the passenger side. Though the driver was kept in the seat by the wearing of his seatbelt his lower leg & foot were wedged between pedal & console causing injury.
THE EMPLOYEE WAS CLIMBING DOWN THE LOADER LADDER AND STEPPED ON A ROCK THAT WAS HIDDEN IN SOME WEEDS. THIS CAUSED HIS LEFT KNEE TO TWIST RESULTING IN A STRAIN. DECLINED MEDICAL ATTENTION UNTIL 9/29/05 - DIAGNOSED WITH A TEAR.
Ee was dumping a load of waste & on his way back to pick up a load of sand he hit a pot hole in road. Ee originally went to Dr on 9/15/05 for exam on this injury & since no observations were made, we considered this as diagnostic only - no lost time or medical treatment. This is a reoccurance of the same injury. EE is now losing time as of 3-13-06 for this injury.
Employee stated while walking down a ramp he slipped and felt pain in his right knee. Employee received conservative treatment until 11/17/05 when he had surgery and began losing time from work.
Employee was checking the belt number on the drive belts and the conveyor started in motion. Employee's left hand went through the nip created by the drive belt and the sheave, resulting in laceration to left index finger and compound fracture to left middle finger.

EMPLOYEE WAS DRIVING A HAUL TRUCK UP A HILL WHEN THE TRUCK STALLED. IT BEGAN DRIFTING BACKWARDS AND THEN ROLLED ONTO IT'S SIDE.
Sitting in the deck chair of the roof bolter and tramming it, he got his head caught between the canopy and coal rib. No information on individual received until 10/5/05.

A contract laborer was assisting cleaning up a lime spill with a shovel and a big bag with several others. After he went home he noticed a burns on the upper inside of his legs. He went to the doctor the next day and the doctor treated the burns and told him to come back in 2 days.
Carpenters were told to carry crates from woodshop to plant. When lifting one crate under 25 lbs, employee felt a snap in his shoulder area.

EE was picking up a load of slurry clay: he had left his truck & was at the base of steps leading to loadout office: where he slipped. The area was wet there was some algae under the steps, but not in the walking area, grating was also in place to prevent slips. He reported that he had slipped. He said that he was not hurt, other than his rear. We learned 10/11/05 he is off work.

EE was wearing his safety glasses. Was performing his bid job as a bagger. Wind started blowing rather hard for the 1st time in weeks & some foreign material got in his right eye irritating it. After 24 hours the redness didn't recede so he was sent to a doctor. Medication is working wonders as the redness is going away as of today 9/15/05. EE missed 9/19-20. Worsened over weekend.
The millwright was shoveling stone off E-4 conveyor walkway when he felt a pain in his lower abdomen.
Working on beltline, belt structure fell and hit his right hand middle finger, mashing it.
EE was bolting in #5 place, installing the outside bolt when he noticed that the top directly behind him began to drop and fall out. He attempted to get out of the way but a piece of rock measuring approx. 3' by 2' wide & 1.5"" thick fell striking him on his left leg behind the knee. He was taken to ER where x-rays were negative. Referred to specialist diagnosed with bruised muscle.
Employee was walking around dozer when his foot sank in soft dirt causing ankle to roll. Employee worked two days before going to doctor and missing work.
Employee was adjusting the jaw crusher w/4' rachet, rachet slipped off nut. Employee fell on top of loader bucket, hitting his leg on the gusset on top of the bucket. At the time of the accident the loader bucket was down on the ground and shut off, bucket was tilted forward w/material behind bucket so it wouldn't move.

Employee had hurt his shoulder on 9-12-05 but had continued to work. Employee Dr. decided on 12-15-05 employee needed some corrective surgery.
EE WAS WALKING AROUND COAL AT FEEDER AND TRIPPED ON LOOSE COAL. HE TWISTED HIS RIGHT ANKLE.
Employee was using a doublejack hammer to drop a liner inside a ball mill after the bolts had been removed. When the liner dropped, the balls caused the liner to roll towards the employee and the liner struck the employee above the left knee.
The ee was struck on left knee by a piece of falling rock.
Three employees were moving a pallet jack from the first floor to the second floor in the prep plant elevator. As our employee and one of the contractors was closing the first set of verticle doors on the elevator, the bottom door caught the edge of the pallet jack lifting it up and sliding off striking the employee's left foot.

EMPLOYEE WAS IN THE #2 RECLAIM TUNNEL, CLEANING OUT A PLUGGED FEEDER CHUTE WHEN A HOSE BLEW, STRIKING EMPLOYEE ON THE SHOULDER.

He stated he was chaning batteries, stepped on a rock , twisted his knee. On Monday, He tried to work thru the knee pain. History of problem with knee dating back to age 18. x-rays taken in the e.r. and interpreted by dr show no new injuries. Visible arthritis in the knee, reported to msha illness.
Dredge operator was sitting in control room of dredge watching gauges when a spider landed on his head and bit him on the top of his head. ** Original online form 234915 **
Employee was changing a ripper tooth on 9231 D11. Used a 2-wheel cart, with the bottom lip of the cart used to pry up the bottom of the tooth to aid in alignment. Once aligned, a hammmer was used to drive the locking pin into place. When the pin set, the cart slid out, causing employee to lose balance & fall back, striking head against truck. Stitches were required.
Employee stepped on a rock, his right knee gave way. He went to the doctor and was returned to work. His knee was still giving him some trouble. A return visit was set up and a MRI was ran. Surgery was scheduled. He was off work the day of the surgery and was off Monday the 7th. Employee's work status is light duty and a return visit is set up for 11/18/05.
Employee while driving a loaded 777B Cat truck backed over the dumping area and the truck over-turned causing serious injuries (broken ribs, arm, back, fractured sternum, collapsed lungs). Employee was airlifted to medical center.

The injured has an inherited degenerative disease that results in bone chips and injuries from lifting heavy objects. He must have surgery to remove the bone chip and rehabilitation before returning to work.
Member picked up a 55-gallon barrel weighing approximately 150 pounds.
Employee alleged that he had suffered neck strain from wearing his hardhat. He then alleged that it was actually a lower back strain from operating CAT D350 haul truck (S/N 9LR00727) during pit haul. Employee reported injury 5 days after injury and had indicated on time sheet that he was not injured on 9-9-05.

Employee was dismounting a cat d-11 dozer when his right foot slipped off the push arm causing him to slip and wrench injured his back.
Employee was getting off forklift when his foot got stuck on the step and he stumbled and hurt his right knee.
Employee felt a twinge in his back while moving a water pump to do repairs. Lost time injury.
Employee was on operator side of bolter. He tried to pull some loose draw rock using the ATRS. The rock broke loose and fell on his left hand cutting index, middle and ring finger. The rock broke when hiting the bolter and knocked him down.
Employee tweaked his knee while moving cable. He stated he started to feel sharp pains in his right knee while moving the cable, and the pain continued when he climbed on and off the truck. He was taken off work until 9/11/05.

Injured stated he was walking near the scalehouse and fell landing on his knee. He had no lost time until 10-5-05. He left work and never returned.
VICTIM WAS REMOVING ROCK FROM TAIL PIECE WHEN A ROCK FLIPPED OUT AND STRUCK HIM IN THE RIGHT ANKLE. THE VICTIM WAS NOT IMMEDIATELY CONCERNED. HOWEVER, THE ANKLE STARTED SWELLING SOON THERE AFTER.
Employee stated that while carrying a bottom belt roller on #12 belt at 109 block 3 west, his feet became entangled with a compressor hose. This caused employee to lose balance and start to fall forward. He caught self. Employee is experiencing pain in left shoulder.
Employee was climbing out of a scoop when his foot hit the directional control lever which caused the machine to move and pinch his hip. There were no fractures or internal injuries as a result of this injury.
He was helping change the screens and as he lifted one screen he felt a sharp burning pain in his left shoulder
The employee was positioning himself in order to tear out a bottom structure stand when he suddenly experienced sharp pain in his left knee.
A rock slid off the top of the shearer and hit employee in the knee while he was tipping the shield to adjust profile. Lost time injury.
While employee was being loaded a rock slid across the load hitting the right rear side of the bed causing employee to be jarred in the truck.
EE WAS PICKING ROCK INSIDE TIPPLE SHACK. HE SLIPPED AND FELL TO FLOOR. HE TRIED TO CATCH HIMSELF AND HE LANDED ON ELBOW.

THE PRYBAR SLIPPED AND HIS HAND GOT WEDGED BETWEEN THE COPPER. HE THEN PULLED HIS HAND OUT & AS A RESULT REC'D A SHOULDER INJURY.
EE stated that he locked & tagged out the power to #94 Coal Hauler Charger & then replaced the receptacle on #3 Battery & receptacle on #94 Charger. He then connected jumper cable to battery & charger. He stated he then was attempting to bolt a fuse to a shunt when an electrical arc caused electric burns to both hands.Result of grounded lead & power from battery
Employee was adjusting ventilation control when he stepped backwards causing a twist in his right knee.
EE WAS ATTEMPTING TO LIFT A ROCK AT THE EAST CORRIDOR TRIPPER ANGLE PLOW AND HE FELT PAIN IN HIS LOWER BACK.

Employee made 4 cable splice, when he stood up his knee popped. Employee started missing work 9-28-05.
THE EE WAS TRAVELING IN A MANTRIP WHEN IT DIPPED DOWN. THIS MOTION CAUSED THE EE'S KNEE TO STRIKE THE ROOF, RESULTING IN A LACERATION THAT REQUIRED STITCHES.
While rolling unused conveyor belt in small rolls strained his back.
Employee was carrying 5 gallon pail of waste/dust on 9/7/05. Employee slipped on oil on the steps while going down. He felt a slight pain in lower back. On 9.9.05, he re-injured back while lifting/reaching forward to position mixer box in place.
EE STATED THAT HE WAS OPERATING A DIESEL LO-TRAC, PLACING A MOTOR IN PLACE FOR A FUTURE BELT HEAD DRIVE INSTALLATION. HE GOT AGAINST THE COAL RIB WITH THE LO-TRAC, CAUSING SOME MATERIAL TO FALL FROM THE RIB. THE RIB ROLL STRUCK HIM ON HIS RIGHT LEG, CAUSING A FRACTURE TO THE TIBIA AND FIBULA IN HIS RIGHT LEG.
EE WAS HELPING LOAD A WIPER BLADE & A METAL BURR ON THE BLADE CUT HIS RIGHT RING FINGER
On 9/7/05 operator was hauling mud in dump truck when he stalled the engine and drifted back about 30'. His right rear wheels ran up the road berm and the truck upset on its left side. He was wearing his seat belt and suffered a sprained ankle and bruise. He was x-rayed and received an ace bandage.
Coworker got Employee's attention & indicated that he had stuck a roof bolt. Employee hurried over to the left (cab) side of the bolter and tried to save it. He ran the head up while holding the roof bolt wrench. When the wrench came in contact with the bolt, because the resin had already set, the bolt/wrench kicked in against the jaws, pinching employee's finger.
Employee states he hit a pothole causing pain in lower back. He went to the doctor for the pain. Employee was newly hired the day before the incident occurred. Prior to his being hired he had medical conditions relating to his back.
WHILE BACKING UP THE WATER TRUCK FROM BEHIND THE JOB TRAILER THE OPERATOR BACKED INTO AN OPEN EXCAVATION THAT WAS BEING DUG FOR THE NEW SCALE HOUSE. SHE BACKED UP DROPPING HER RIGHT REAR TIRES INTO THE EXCAVATION CAUSING THE WATER TRUCK TO OVERTURN (ROLL OVER) INTO THE EXCAVATION.
