Accident type, without injuries

In the 4450 the back came down at the intersection of a load center. The affected area was estimated to be 25 feet X 20 feet X 2 to 5-6 feet deep. The fall did not impede ventilation or passage in the drift. Only access to the load center.

Accident type, without injuries

A roof fall occurred on the 3rd 54 belt line 3rd main east xc 66 #5 entry. The fall measured 110'x 19.5 x unknown and was caused by a hidden slip in the mine roof. The area was mined in May of 2008. There were no injuries, no equipment involved and ventilation was not affected.

A fall of roof occurred in the 7th Southwest Sub-main 31 crosscut, Entry # 5. The fall measured approximately 39 feet long x 30 feet wide and height is unknown. The area was supported with 6 foot fully grouted bolts.

A unplanned roof fall occurred at x-cut 42, in #4 heading at old 7 south. The fall was about 6 foot high, 20 foot wide, and 60 foot long. The fall timber and danger off.

An unintentional roof fall occurred on the #3 Section spad 16486. The fall was approximately 20ft x 20ft x 3-5ft. There has been a roof fall clean up planned developed and approved. A Joy 14/15 continuous miner was involved in the roof fall. No injuries were reported.

A roof fall above the anchorage point occurred in the 2 East Longwall tailgate at engineering station 96+26. The fall was 67 feet long and 12 feet high. The fall occurred due to a weak roof structure of stack rock. It didn't involve any compliance issues, it did impede travel. No equipment involved, No other part 50 criteria applies.

3200 Chippy Hoist low voltage - Mode Break Switch tripped, Hoistman tried to reset, electricians were called, reset and reboot computer up and running at 1246 hrs.

Unplanned roof fall at #61xc, #2 entry, ft. #9068 on #4L travel road. Fall occurred in intersection supported with 84"" tr bolts and 12'cable bolts. Fall dimensions 19' w x 19' L x 8'5"" h. Fall did not cause injury, impair ventilation, or entrap miners. Fall area developed 7-1-2015.

A roof fall was discovered in the 2 North Right Return at Brk. 16. The fall was approximately 40' long and 10' high extended from brk 16 toward brk 15. All approaches to the fall were breakered off.

An unplanned roof fall occurred at the 1585' mark in the 10 west tailgate entry number 2. The area had been mined in October 2012 and the fall occurred in a three way intersection. The area had previously been supported with 8' torque tension bolts in t-3 channel as the primary roof bolt with 12' cable bolts between the existing rolls of the primary bolts.

A roof fall occured in the #7 Switch on the Crabapple slope bottom. The fall occured in a 3 way intersection and measured approx. 25ft long by 12ft high by 25 ft wide. There were no injuries as a result of this event.

Elevator indicated a fault during a dry run. All employees were notified to evacuate and fault was cleared and elevator was inspected and back in service at approximately 10:10am on 8/24/15. No persons injured or trapped.

A unintentional roof fall was noticed by mine personnel. No injury or equipment damage occurred, with no interruption to the ventilation system. The roof fall measured approx 25 feet long and 17 feet wide, falling aaprox 2 feet above the anchorage point. The roof fall area was immediately dangered off and supplemental roof support set around the perimeter of the fall area.

Welding on 475 S/N 20052 on the pivot shaft area. Got hot on the inside and started a fire on the inside wiring that could not be extinguished.

At approx. 11:07 AM, a roof fall above the anchorage was discovered on the North A belt line. The fall was approx 30'long x 17.5' wide & 6' high. This area had been mined in Oct. of 2005 & was supported with 6'fully grouted bolts. There were no injuries due to this accident. The area was re-supported and then cleaned up. Weathering may have been a factor causing this roof fall.

The elevator at Cumberland #9 Portal went down at 8:00 AM due to broken guide rollers. There was no one on or trapped in the elevator at the time of the outage. The elevator was down for 2 hrs 10 mins.

Two workers were cleaning the inside of the #6 boiler scrubber ducts. EE took a step back to get out of co-worker's way when he stepped into the drain piping being used to flush material from the scrubber ducts. Because of the angle that his foot entered into the pipe, he was unable to remove it for about an hour until a section of pipe was removed. No injuries resulted.

A reportable roof fall was found during the weekly examination in Sly Gulch Return, Entry 3, crosscut 10. The fall was approximately 45' long, 27' wide and 7' high.

As a Bolter operator was installing chain hangers in U15 83 panel C tunnel he began to see visible emmisions exiting the hole he was drilling. The unit was evacuated, power was dropped and a supervisor was called to check the air. At Approx 1:15 a supervisor performed a legal gas check at the hole and registered .45 % CH4. The Mine superintendent and MSHA was notified.

On 08/18/2015 a sill failure in the 4260-135 occurred as a result of a fall of ground in the inactive 4200-135. A sill failure is not reportable, the ground fall did not occur in an active area and there were no injuries therefore a call was not immediately made to MSHA. On 11/16/2015 an order was issued for not reporting the event within 15 minutes.

Employee was riding in the Fine Crusher elevator when it stopped. Elevator repair personnel were on site and were unable to get the elevator to move. The employee was removed from the elevator car after a piece of tin was removed from the elevator shaft. The employee was uninjured.

Roof fall occurred on old 6B-Belt @ 5 BLK. Fall was discovered by weekly mine examiner and MSHA inspector. Area was previously bolted with 6' resin bolts on a 4'x 4' pattern and two (2) 16' cable bolts spaced every other row. Dimensions of fall are approximately 16'W x 18'L x 20'W.

Hot work was performed on the fan during first shift, fire watch was performed with no issues. All employees exited the mine at 7PM. At approximately 11:45 pm third shift maintenance employees discovered the floor around the fans producing smoke. The employees used two extinguishers on the floor and called for a mine evac. No flames were ever observed.

A non-injury (unplanned) roof fall occurred at this mine. The roof fall occurred on the 2D Belt, at crosscut 17 in the #6 entry through crosscut #21 at approximately 8:40 p.m. A 103(k) order was issued. Please see citation # 7278196.

on 8-17-2015 a roof fall was discovered in a weekly air course. The area was initially bolted with 8 foot point anchor resin assisted roof bolts. the fall is approximately 25' long X 17' wide X 9 Feet in height. Weathering of the roof was determined to be the cause of the accident.

A unintentional Roof fall has occurred on the #4 Unit in the #5 entry at the 16xc. The fall was approximately 24' long by 20' wide and 10'-12' thick.

A roof fall was found on 8/16/15 at 6:30 AM in a four way intersection at XC 65 entry 4 main west. Area had been mined July 21st 2009 and supported with 4-6' full resin bolts. No injury resulted. A slip above bolt anchorage was reveled and likely contributed to failure. Fall measured 54' longX30' wide X15' high.

An unplanned roof fall occurred on the north east mains, #2 belt break #27, #4 to 5 entry survey station #02978. Area of fall was developed 1-4-11, and supported with 48"" resign bolts, it is 6600' from active section. Fall measured 75' wide x 19'L x 4-8'H. Passage of persons was affected, ventilation was not affected, fall area will be rehabilitated.

Power Outage - Lost power to the mine there by shutting down the secondary escape way. No injuries of other issues. Power was restored at 2:25 AM and the mine was reentered at 3:00 AM.

A non-injury (unplanned) roof fall occurred at this mine. The roof fall occurred on the Main Line #4 Belt at crosscut #189 near spad #3121 at approximately 6:30 p.m. on 8/15/2015. A 103(k) order was issued. Please see citation # 7278195.

A roof fall occurred in the #9 coal seam in the 2nd Southwest Mains at crosscut 34 in the #8 Entry. No injuries or ventilation short circuit occurred as a result of the roof fall.

A unplanned roof fall occured in the crosscut between 2 and 3 entry at 44 x-cut in the 3L section. The fall measured 16ft wide 10ft high and 50ft long. No injury or equipment damage as a result of this incident

While on weekly run, examiner found a fall of roof above the anchorage point. This fall is located 80' inby survey spad 1317, #1 entry, 1 crosscut outby black mains inlet. The area does not affect air or travel through the return area. 7'H x 40'L.

Unplanned roof fall at the 47 block intersection of 4 belt, extending out towards the track. Fall approx. 20' high, 20' wide and 30' long. Small pocket of unconsolidated moist slick rock discovered above anchorage during investigation. Fall was cleaned up and re-supported with bolts, cribs and floor to roof support.

On 8/11/15 a roof fall was discovered on 3 West Mains at Brk. 28 in the number 9 entry.

Roof fall in the crosscut #16, between entries #1 and #2 of the Main West section. Dimensions of fall are approximately 17 feet x 18 feet by 8 feet high.

A grass fire was discovered in the tall grass above 7&8 North Pre-strip area. The fire was not extinguished within 30 minutes. A motor grader and mine firefighting equipment extinguished the grass fire. No miners, mine equipment or structures were ever at risk during this fire event.

On 8/11/2015 a fall was discovered on the 3 West Mains at Brk. 14 in the number 9 entry.

Electrical problem, MSHA was notified at 5:13 p.m., evacuation started at 5:00 p.m., completed at 5:13 p.m. Wire was repaired at 8:15 p.m., miners returned underground at 8:35 p.m. The hoist was tested before put back in service.

The elevator was removed from service due to an electrical issue with the door. The elevator door was repaired and the elevator was returned to service.

Two shaft contractors working in the 14 Ft. shaft signaled the hoistman to return to surface from a work location at the -540 level. When the hoistman began to move the conveyance he discovered the 14 Ft hoist was inoperable.

A Roof Fall (20' long X 10' high X 18' wide) was found in the 6-North Left Return,# 1 Entry at 21 Block spad number 8711. There were no persons injured, ventilation not disrupted but the walkway is obstructed. No equipment was involved and no other 50.20 criteria applies. The fall will be supported and walkway reestablished.

The #1 Portal Elevator was reported out of service at 1:51 AM. There was a processor fault and had to be reset. Mon Valley Integration has been contacted to re-program processor. Elevator back in service at 2:51 AM.

A fall was discovered during a pre-shift exam on the morning of 08/09/2015 by the examiner, it was called in as soon as possible. The cause horizontal stretch in the area. That was determined at a later date.

The #1 Portal elevator was reported out of service at 10:06 PM. There was a processor fault and had to be reset. The elevator was back in service at 10:49 PM.

A transformer caught fire on a bank of transformers along the mine access road just prior to the 6 Left Shaft hallow at the surface area of the Federal No. 2 Mine. The transformer fed power to a sedimont pond pump. The fire at all times was contained to inside of the transformer and allowed to burn itself out.

A fall of the roof occurred on the 2nd N.E. Mains, 3rd North Belt Line,#4 entry, crosscut #4. Fall measured approximately 16' width by 20' length by 8'-12' height.

A roof fall occurred in the West Lease Bleeder #2 entry at #16 x-cut. The roof was bolted with 7' resin bolts and trusses. No person was injured as a result of the fall. The high voltage electrical cable was pulled down by the fall and re-routed prior to any rehabilitation work being done.

A roof fall occurred on 3-North-1 Mains, #2 entry, crosscut #9. The fall measured 20' by 20' and was 9' in thickness. The area was originally supported with 5' Hi-Tech bolts, truss bolts, cribs and beams. There were no injuries and the escapeway was re-routed. The fall will be cleaned up and re-supported.

During an inspection by MSHA inspector at the escape hoist located at crosscut 89 on west main it was found that the AC output card was bad.

Carrier had 2 technicians enter the Crusher 3 cargo elevator, elevator stop in between floors, crusher 3 control room called electicians, they reset elevator both carrier technicians were fine no injuries or medical assistance needed, they were in elevator approximately 25-30 minutes.

A roof fall was found in entry 6 XC 70-71 west intakes. Area was mined in August of 2009 and had been supported with 4-6 ' resin pins. No injury or property loss occurred. There are no plans to clean up fall. Travelway is being rehabbed in entry 5 and will serve as the new primary escapeway. Fall was approximately 35' long X 20' wide X 8' high.

There was a possible ignition of coal dust in the electrical cabinet of the 18 Shovel when the shovel was restarted after an electrical fault.

An Unintentional roof fall occurred on the 3rd North Travelway. This happened between 18-19 crosscut.

A process attendant was in the preheat elevator traveling to the 10th floor when the elevator car stopped between the 6th and 9th floor. Process attendant became entrapped in elevator at 2:40 PM and exited at 4:00 PM. Elevator technicians arrived at plant to troubleshoot and adjust the governor in the machine room. There were no injuries as a result of this incident.

An unintentional roof fall occurred at approximately 1:15 AM. The fall occurred at the long bottom seal at the intersection of the No. 3 seal. No injuries occurred nor was ventilation impeded. The travel way to the No 3 seal was blocked. A plan has been submitted for the clean-up of the fall. The fall measured approximately 15' long X 15' wide X 3' high.

Upon investigation it was determined that this was not a reportable roof fall . Rock material dislodged from the roof along the travelway creating an area of unsupported top of approximately 30 feet . The dislodged material was not above the anchorage point of the initial support. The area was cleaned and additional bolts were added before the weekly was due .

While cross-ripping a 3:1 slope on Cell 15 leach pad, a Caterpillar D8 dozer encountered a rock which caused the dozer to roll over approximately 120 degrees onto the right side of the dozer. The operator was removed from the cab in less that 30 minutes and there was no injury to the miner. The miner was evaluated at the hospital as a precaution and released to full duty.

Power Plant security notified mine personnel of a fire located between C ramp and county road 235. Upon arrival to the area, supervision discovered a large brush pile burning under power lines. The local fire department was notified. The fire lasted approximately 1 hour and 20 minutes. The source of ignition is unknown.

Unplanned roof fall in 21W discovered reportable at 2:56 am in the #2 entry between 3 and 4 blocks. The fall was approx. 16' wide, an unknown length and width, stopping about 5' into the #3 block intersection. Operator is retraining roof bolters at the mine and has submitted roof control plan changes.

An unintentional roof fall has occurred 8 breaks outby spad 4225 Horse Creek 4 Belt break 107. The roof fall was approximately 18ft x 18ft x 6ft. There were no injuries reported and no equipment involved.

An unplanned roof fall occurred in the crosscut between the no. 4 and no. 5 entries at break #25 on #4 belt. (Approximately 20 breaks outby the face location). The roof fall measured approximately 20' wide, 14' long and 3'-3.5' thick.

Loader operator was pulling out of a parking space next to an area where employees were parking their cars. This area was tight and the operator scraped along the side of another employees minivan, damaging the sliding door and rear quarter panel. There was no one else in the area and there was no injury. The minivan is drivable and repairable.

Found roof fall in the no. 1 entry approx. 400 ft from the face. Fall occurred in the intersection. 20 ft wide, 20 ft long, 6 ft thick. This is active non-producing status. No coal being produced at this mine. No one was in the mine when the fall occurred.

A Roof fall (40' long X 8' high X 20 to 28' wide) was found on the Main North Belt at 150 block spad number 6631. There were no persons injured, ventilation not disrupted but the walkway is obstructed. No equipment was involved and no other 50.20 criteria applies. The fall will be supported and the walkway reestablished before mining resumes.

A fire was discovered near the Turlington pump yard. The fire began off mine property on a nearby county road and traveled onto the mine site. The fire lasted approximately 33 minutes. The source of ignition is unknown.

Struck by falling object

Was putting screw jacks in to hold a pony boom up, supports dislodged and the pony boom came down hitting shoulders knocking EE to the ground. *On October 1, 2015, the injured miner died. On October 13, 2016, MSHA's Fatality Review Committee determined the death to be mining related and chargeable to the mining industry.

Employee was operating a jumbo on an incline in the main haulage. The operator drifted to the right rib and the spare drill steel in the drill steel rack came in contact with the right rib. The drill steel gained pressure breaking the drill steel rack striking employee with the drill steel in the head causing a fatal blow.

Struck by rolling or sliding object

Associate backed the haul truck under the conveyor to load Mineral filler. Preliminary findings indicate that the associate climbed to first level platform under the silo hopper. While standing on the platform, the upper section of the hopper detached from the silo. The entire hopper, along with contents, then collapsed onto the platform, engulfing the associate.

Struck by rolling or sliding object

There were no witnesses to the accident. The operator was found outside of his loader buried under gravel that had sloughed off of the stock pile he was loading from. Fellow employees were attempting to remove the gravel from on top of him when officials showed up. Officials declared him dead at the scene.

Caught in, under or between running or meshing objects

EE was cleaning the JSeal pump at end of shift and removed the grating of the hopper. Right hand came in contact with a moving auger. EE suffered traumatic amputation of middle and ring fingers on right hand.

While the offside operator was bolting the slab the IE noticed a damaged roof bolt in the entry. IE decided to spot a bolt beside the damaged bolt. IE finished drilling the hole, inserted resin and bolt into the drill hole, started to push the roof bolt up into the hole, he felt a pinch on his finger. The IE received sutures to close the wound to the end of his finger.

EE was holding on to the cable for support and was using his foot to get the screen unstuck. When the screen swung around snag the cable pulled his thumb into pulley and I think he yanked his hand out leaving the tip of his thumb to the first joint in the pulley. We backed the cable down, put the thumb on ice and took ee to ER.

Struck against stationary object

The employee was performing the task of removing belts on the A.L. Fireroom hazardous bag house. The mechanic positioned a screw driver against the sheave and was prying the belt to remove it, at which time the screw driver slipped off the sheave resulting in his hand coming in contact with the pulley severing his left ring finger.

Employee was cleaning a plugged cyclone chute in Screen Plant. Employee reached into an open access hatch and finger got caught in Rotary Air Lock Valve. Tip of Right middle finger was amputated.

Struck by falling object

Victim allegedly said rock hit him on top of his hard hat 40' inby spad then trammed pinner 80' to last open x-cut. Help hang pinner cable & got pinner out of way. Then he told co-worker what had happened. Co-worker escorted him to section foreman then escorted him outside, then took him to hospital. He refused an ambulance.

Inhalation of radiations, caustics, toxic and noxious substances

Employee inhaled gases from an arc flash that occurred at the power center.

Over-exertion in lifting objects

Employee was stacking 50 # bags of product onto a pallet. Employee stated that he had turned wrong to place the bag on pallet and had a sharp pain in his right shoulder blade that went to his neck and shoulder with temporary loss of breath and face/arms and hands were tingling.

Over-exertion in lifting objects

A fully trained miner was lifting a 10 ton jack that weighs approximately 25 lbs. out of a box to put on a mantrip and strained his lower back. The area was dry and flat level surface .Employee did not use proper lifting technique proper PPE was not a factor.

Struck by falling object

Individual was assisting in hanging 12470 cable on the Longwall Power move when the cable slid off the pump car creating a chain reaction of the cable. Because of lack of concentration the cable struck employee on right shoulder. There were no compliance issues or equipment, did not involve lack of training or attitude. Employee continued to work until 9/19/2015.

Employee was pulling a shield in on the longwall face. In the process of pulling in the shield his foot caught got under the pontoon of the shield and it caused him to smash his right foot. It broke the tips of three of his toes and the employee had to receive four stitches in one of them.

EE fell when he stepped off of the bottom rung of a Baghouse ladder resulting in a broken metatarsal bone in his left foot.

Inhalation of radiations, caustics, toxic and noxious substances

Employee inhaled gases from an arc flash that occurred at the power center.

Over-exertion in pulling or pushing objects

Employee stated: was moving power on #2 unit and was in #6 entry moving power cables. There was mud and water in this entry and was dragging cables thru this area resulting in straining right side of lower back.

Struck against a moving object

A fully trained employee was riding in a 5 man mantrip leaving 77 wall going inby on 2 track when he traveled past 80 wall he jumped the switch and struck another ride parked in the spur resulting in a strained back PPE was not a factor.

Over-exertion (Not Elsewhere Classified)

Employee was changing roller in a tail piece and strained their back.

Struck by falling object

Employee was changing a rear tire on a water truck. The area was wet. When the employee used a lifting bar to move the tire the bar slipped on the wet ground causing the tire to fall on the employee's foot.

Watering roads in quarry, dust or dirt in left eye.

Over-exertion in lifting objects

A Calcium Department Employee was vacuum cleaning up lime that had accumulated below the briquetting area. When removing the lid of the vacuum system, lime dust particles were released into his face and into his eyes. They had just loaded a winch power box into the scoop bucket and they were going to take to the mouth of the section. In the process of walking around the scoop three rocks fell from the mine roof and drove the employee down to ground beside of the scoop and into the mine rib.

Employee was in the process of unloading trash from bucket of scoop. The scoop was loaded with mine trash and was parked near the dumpster on the surface of the underground mine. The employee was lifting trash into the dumpster. He pulling on piece of curtain it was stuck, resulting in twisting of lower back. Continued to work rest of shift then went for medical attention.

Walking by miner in 1 East #3 heading inby 7 block and a piece of loose rib (2' by 8-12"") fell and struck him in the head and neck. X-rays were negative, given time off for possible soft tissue damage. No problems with training, ppe, mining machines or compliance issues.

Employee was pushing a ram car up a grade with a diesel scoop. The ram car gained traction allowing the bucket of the scoop to lose contact with the ram car. Employee was using a pocket knife to cut a zip tie. While doing so, the knife slipped and punctured his hand. This resulted in stitches from the local emergency room. He returned to work, but was instructed to follow up with a hand surgeon for possible nerve issues. At this time, he is on restricted duty, but there has not been any lost time.

Once at the location the employee jumped a fence once to retrieve the barrels and hand them over the fence to his supervisor. When they were done with the barrels the employee jumped back over the fence (approximately 4.5"" tall) and fell forward onto the ground, landing on his right wrist.

Employee was on an 8-foot step ladder replacing v-belts for the VSI, when stepping off ladder onto the ground, employee states he twisted his right knee and ankle stepping on loose 3"" rock. The employee sought medical attention on 9/4/15 diagnosed with a knee/ankle strain and given restrictions.

Operator was climbing an extension ladder to secure it when it slipped; the operator remained on the ladder while it slid downward, injuring his right wrist. Two hours later he was taken to the hospital where an examination was performed and it was recommended by the physician that the EE be given restricted duty.

EE's were conducting routine maintenance tasks on rail car. While changing a brake shoe on the rail car, EE heard air and felt his right ring finger become caught between the brake shoe and the brake beam. The second EE was able to use a pry bar to free the EE's finger. EE suffered an avulsion fracture to the right ring finger and was released to light duty restrictions.

The employee felt discomfort to his left shoulder when the bucket contacted with the pit floor. We were informed on 9/10 that he was going to seek physical therapy.

While drilling a hole for a roof bolt, a piece of stone broke off and struck the employee on the outside of the left leg bending the leg to the side. The employee continued to work but he had surgery done on his left knee on 11/24/2015 due to the injury.

The employee was moving boxes of copy paper in her office and felt pain in the lower back left side. She advised her supervisor, but did not seek medical attention. On 9/1/15, the employee was still in pain so she went to the doctor and was advised she sprained her L-26 lower lumbar.

Float Crew was changing out a 7' long 20"" exp. joint on matrix pump. Bolt holes were not lining up on flanges. Float Crew Leaderman(FCL) used a lifting strap as choker around exp. joint. He cut one side and when he had the second side cut almost halfway through the conveyor chain broke, (due to it having tension on it), and flew back striking employee on his left hand causing a laceration requiring stitches to close.

Eval 08/26/15 lifting restrictions of no > than 10 lbs. Employee had been placed back to normal role prior to 08/26/15 evaluation.

Employee had came into my office on 8/27/2015 explaining to me he had injured his finger the week before and the swelling had not went away. He was lifting up stones and placing them down, he had got his finger caught under one as he was placing it down causing an injury to his right pinky and ring finger.

On 8/21 employee sustained a puncture wound while using a hammer and punch. On 8/25 he was complaining that there was a piece of metal in his left hand and taken to the doctor. The hand was x-rayed and a piece of metal was. He was sent back to work with NO Restrictions. On 9/2 he had the piece of metal surgically removed and placed on restrictions with limited hand use.

Employee was in the process of removing the engine from 12019 hauler. The employee was using a pocket knife to cut a zip tie from a wiring harness. The employee was cutting in the direction of his left hand and the knife contacted the top first knuckle of the employee's hand causing a laceration

Struck by falling object

A WEDGE ON A PIVOT VICE BROKE, WHICH WAS MOUNTED ON BED OF SERVICE TRUCK. WHEN EMPLOYEE WAS REMOVING THE AIR HOSE THE VICE FELL ON EMPLOYEES LEFT PINKY TOE. THIS RESULTED IN A BONE FRACTURE.

Fall from machine

The injured was working on a company service ride air compressor when he lost his footing injuring his right arm.

Employee was drilling in 4 tunnel when another employee pulled up in a skid steer to discuss where to clean next. He was sent to 5 tunnel. As the employee turned the skid steer to exit 4 tunnel, the skid steer boom pinched the other employees ankle in between the two pieces of equipment.

Accident happened in equipment yard when jumper cables slipped from miner's hand making contact with miner's face area creating a gash near lip.

Over-exertion in lifting objects

EE was switching the Oxygen bottle on the torch cart. He lifted the bottle onto the cart and he felt a pull in his back just below his shoulder blades. He did not go to the doctor until the following Monday (08/17/2015).

Employee was helping another employee remove a 6 foot long piece of channel steel from a crate. When the other employee set his end on the ground, the channel on the injured employee's end got caught on the crate. As the employee attempted to dislodge the steel, it broke free and mashed the employee's left ring finger causing a laceration requiring nine stitches.

Two employees were unloading 4"" I-beams to use as roof support. One employee picked up one end of a beam causing it to roll over pinching the injured employee's finger against the bed of the flat car.

An employee was leaving at the end of shift when she tripped on the last step causing her to land on her knees and hands.

Employee shoved troughing idler through gate on aerial lift that was being utilized to transfer material and idlers from 1308 conveyor. Lift was lowered to with 6' to 8' of the ground and idler was shoved out by employee. Employee was struck on the right ankle by troughing idler. Ankle was significantly sprained and employed was placed on restricted duty with medication.

Fall from ladders

Employee was climbing up a 7ft 6 inch stationary ladder and missed grabbing second to the top ladder rung. Employee fell approximately 3 ft. 6 inches to the grating deck.

Struck by falling object

Employee changing the feeder oil on power screen jaw crusher. While taking off the large feeder pan for inspection, the pan slipped from hands and injured employee's finger.

Injured Employee (IE) was moving/lifting a mobile stand so he could remove a screw trough cover to replace a broken dust collector screw. The stand got hung up so he lifted it a little and it shifted slightly down instead of up, as expected, and pinched his right thumb tip between the stand and the screw trough cover. Result was a fractured thumb tip and nail avulsion.

Employee was cleaning the kiln riser using the Woma Lance. As he pulled trigger, a pin hole in the barrel allowed the high pressure water to shoot thru ee's glove.

Employee pinched finger between railing and pipework. Per email from reporting party, EE was changing out the bag on the number 3 zimpro using a pipe wrench to hold bag up, while coworker, (new to job) started loosening the fitting. Injured EE's finger was caught between handrail and pipe wrench.

A company mechanic was replacing the starter on a Cat 730 articulated haul truck. When he removed the bolts from the belly pan it fell smashed his left index finger. He had a laceration about 3 inches long on his finger. He was taken to urgent care where he received 9 stiches.

The EE was replacing drive belts on the sand screw. He was standing at the end of the screw when he reached down to unscrew the bolt on a guard. He felt a pop on the left side of his rib cage. He reported it, but did not feel the need to go to the doctor until 8/13. He received a shot for pain on 8/13, and another for inflammation on 8/17. Released to full duty on 8/20.

Employee was tightening valve packing, wrench slipped off nut and crushed finger between valve and wrench causing a tuft fracture 4th digit left hand.
