A roof fall was discovered at the beginning of the shift on the main line belt conveyor 11 crosscuts inby the portal. The fall measured approx. 20' long by 8' wide and 6' thick at its peak. The fall was removed and the area was supported with cribs and cross collars.
BACKHOE OPERATOR WAS LOADING CRUDE CLAY TRUCKS OFF OF THE TOP SHELF (APPROX. 8 FEET) WHEN HE GOT THE BACKHOE TOO CLOSE TO THE EDGE AND THE MACHINE SLIPPED OFF ON THE RIGHT SIDE. WHEN ATTEMPTING TO RELOCATE THE BUCKET TO BETTER SUPPORT THE MACHINE, IT SLIPPED DOWN VERY SLOWLY APPROX. 1 FOOT AND THEN SLOWLY ROLLED OVER ONTO ITS RIGHT SIDE.
Unintentional Roof Fall @ Spad # 1291. 20' (L)x 20'(W) x 5'(H).
SHIFTER WAS MAKING HIS WORKPLACE INSPECTIONS. HE NOTICED A FALL OF GROUND AT THE HGB 3240 PANEL BL. THIS IS AN INACTIVE HEADING. NO ONE HAS WORKED IN THE PANEL FOR 12 TO 14 DAYS.
#2 UNIT, #6 ENTRY, 2ND OPEN FROM FACE. LENGTH 60', WIDTH 18', THICKNESS 6'.
THE NUMBER 1 HOIST FAILED AS THE RESULT OF A BROKEN COIL WIRE. MINERS WERE EVACUATED AT 6:27 AM AND THE EVACUATION WAS COMPLETED AT 6:38 AM. THE BROKEN WIRE WAS REPAIRED AT 7:15 AM, THE HOIST WAS TESTED AND THE MINERS RETURNED UNDERGROUND AT 7:20 AM.
At approximately 11:20 pm in the 1st South Pillar section, MMU 004-0 the roof fell on top of the continuous miner. There was a combination of top coal and rock that fell on the miner causing it to be trapped. MSHA was notified and investigated. The continuous miner was recovered with minimal damage by 7:00am on 12/30/2005. There were no injuries.
Roof fall on main northwest and return entry #1 4 & 80. Continuation of fall in entry #2.
At 10:50 PM on Dec 28, 2005 a rock burst (outburst) occurred in the 4000-226 east stope. Damage consisted of 25' of rib that will require scaling & re-bolting. Damage is 200' from the 226 raise & 30' below the 3700 track level. The event occurred when the night shift rounds were blasted & was likely triggered by distress holes that are drilled & blasted with the rounds.
Roof fall on main south a beltline 30 to 31 crosscut. #3 entry 19+20+19x40 fall was 65 ft long 19 ft wide and 6 ft high.
Had an unintentional roof fall on #9 bolt from #11 to #12 breaks. Fall is 60' long x 20' wide x 5' high.

An inundation of gas occurred on MMU 001-0 Section. While advancing Panel #16 cutting Gob-I on across from #6 right crosscut at #27 break. East Mains sealed area was cut into at #27 break on panel #16. This was located approx 100' from #6 entry survey station #3325, w/last 25' unsupported. No injuries reported, temp vent.controls constructed & seal built according to plan.
the elevator electrical controler malfunction in mid trip causing the otis elevator to stop. 10 employes were stranded for a period of 2 hours. the levator was lower to allow egress of the men. the elevator was removed from service immediately until repaired.
A roof fall occurred in the belt entry #3 ont he 005 section 5 west mains. The fall was located in an enter section 1 crosscut inby survey staiton. 24576. The fall measured 18'l x 14'w x 8'h. The fall will be cleaned up. superbolt will be installed prior to starting clean up operators.
A roof fall occurred on the 4B panel in entry #5 at crosscut 5, spad #25600. The cause of the fall was weak laminated shale. The fall blocked the 4B roadway to #4 unit. Production was halted on the section, the roadway was re-routed and production resumed. The fall area was timbered off.
An elevator malfunction occurred at the Justice Mine due to a bad door switch and brake.
Unintentional roof fall measuring 60'(L) x 19'(W) x 5'(H). Discovered on 12/26/05 in # 2 Entry inby spad # 578.

DURING A NORMAL ROUTINE MAN TRIP AT SHAFT #2, THE HOIST 4703 TRIPPED OUT BELOW THE 900' LEVEL. THE HOISTMAN RESET THE HOIST. THE HOIST TRIPPED OUT SEVERAL MORE TIMES BEFORE ARRIVING AT THE COLLAR. ELECTRICIANS WERE CALLED. THE ELECTRICIANS PUT THE FIELD MOTOR ON A NEW BREAKER. HOIST CHECKS WERE COMPLETED. ALL O.K.
The number 1 hoist failed as the result of a broken wire. Miners were evacuated at 1:20 p.m. and the evacuation was completed at 1:38 p.m. The broken wire was replaced at 1:45 p.m. The hoist was tested and the miners returned underground at 1:55 p.m.
Roof fall occur at a 4-way intersection in No. 3 entry near spad No. 55, 1150 outby pillar line. Water seepage is present and the fall was 50 ft. long by 20 ft. wide and 6 ft. thick.
A roof fall occurred in the 1 East Panel, crosscut #98 in the pillarline between #5 entry and #6 entry measuring 30ft. x 18ft. x 5ft. Five foot fully grouted roof bolts were used to support the area. An undetected slip and kidney bed was determined to be the cause of the accident.

There was a roof fall in the #3 entry, neutral, approximately 20'L x 25'w x 6'.

A methane ignition occurred in the north main section #1 entry face while cutting the miner ignited a bleeder. the flame burned 1-2 minutes and was extinguished with fire extinguishes.
A roof fall occurred in the #6 entry at #13 crosscut in the 1st West Panel off the #11 North Main Panel. The fall originated in an intersection and fell in a North - South direction. The size of the fall was approxiimately 80' X 19' X 8' high. The fall was approximately 2500' outby No. 1 Unit.

AN UNINTENTIONAL ROOF FALL AT THE TAIL AREA OF 6-N LONGWALL FACE. MADE TAILGATE ENTRY TERMPORARILLY IMPASSABLE. FALL OCCURRED IN #1 ENTRY OF 5-N BETWEEN #7 & #8 INTERSECTION. 28'LONG X 15'WIDE X 6'HIGH.
The elevator at the Justice Mine was down from 2:10 pm to 3:10 pm due to a malfunction with the control circuit.
In the area where the guies were knocked out, it was noticed that too many holes had been drilled over time while doing shaft repair, causing a weakness or causing a split zone in the guides. As the loaded 24,000 16 skip travels thru this area the horizontal forces exerted could split the guide along the area where too many holes were drilled.
A roof fall occured int he 3rd west, #7 entry, 3780' station. The fall was approx. 20' long, 30' wide and 8' tall. The area was bolted with 6' and 8' point anchor bolts. The fall didn't affect ventilation, travel, damage equipment or cause any injury.

SLOPE HOIST INCIDENT - On Monday, 12/19/2005 at approximately 11:30 am, ice in the shaft broke loose and hit the buffer switch. The cage was fully operational at approximately 01:25 pm on Monday, 12/19/2005.
A ROOF FALL MEASURING APPROXIMATELY 18' W X 50' L X 5' H OCCURRED IN #6 AND #7 ENTRY IN INTAKE AT SPAD #1928. AREA WAS BOLTED WITH 5' BOLTS.
A roof fall occured in the 1 Main East, intersection of #5 entry at crosscut #60. The fall measured approximately 25'x18'x9'. The area was supported with 6' tension rebar bolts and supplemental support consisting of five (5) 9' mega bolts. An undetected slip was determined to be the cause of the fall.
A gob fire was found in room 000 inby survey station 21+84 in 6LMS by examiner doing weekly exam. Upon notification of smoke the mine was evacuated. Props were set to access area. Water was used to cool fire and it was loaded out with scoop and spread out and extinguished. All hot spots were out at 9:35 PM. Area will be monitored for next 24 hours.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 2ND MW INTAKE XC60 BETWEEN #6 & #7 ENTRY. APPROX. 38'LONG X 9'HIGH X 17'WIDE. THIS AREA HAD LAMINATED LIMESTONE & HAD BEEN BOLTED WITH 4' & 6' RESIN BOLTS. THIS AREA HAD BEEN CRIBBED & DANGERED OFF PRIOR TO THE FALL FOR BAD ROOF CONDITIONS.
AN UNINTENTIONAL ROOF FALL OCCURRED IN INTERSECTION OF OLD 3D TRAVELWAY. THE FALL WAS 20'WIDE X 45'LONG X 10'HIGH. 48""FULLY GROUTED BOLTS WERE USED IN THIS AREA.
A roof fall was discovered @ 10:05 PM on 12/18/05. Fall was in belt entry of MMU 064-0. It was confined to the belt entry & there was no damage to ventilation controls or passage of personnel. There were no injuries. Fall was located @ 25+35 & it fell on the FCT low belt framing outby the tailpiece. Fall was approx 70'L x 20'W x 8'H. Area was supported w/6' resin bolts.

A roof fall occured in the #2 entry of the 1st Northwest Mains at x-cut 12 ( spad # 9+69 ). The fall was approximately 18'W X 24'L X 8'H.
A non-injury roof fall occurred on the 8th west belt line at x-cut 31 in the #1 entry 200' out by the longwall face. The fall was 40' long x 18' wide x 6' to 7' high.
7c track entry break 11 roof fall 6 ft high 30 ft long 18 ft wide. No injurys were reported as and result of this roof fall.
ROTARY KILN IN ZFP @ MINERAL SEPARATION PLANT EXPERIENCED MINOR RAPID COMBUSTION/EXPLOSION DURING SHUT DOWN PROCEDURE. MINOR DAMAGE TO DISCHARGE EXHAUST COWLING, DUCT WORK & SUPPORT PIERS.
An unintentional roof fall occurred in the main east travelway adjacent to mine shaft bottom. The area was originally mined in 1977. Fall occurred due to a hidden slip just below the anchor point. The brows will be reinforced, fall area will be cleaned up and the area supported. The fall area measured 20' x 20' x 5'.
As the night shift crew was exiting via the decline, they encountered a slough of ground from one rib 22'W x 12'D x 10'H. The toe of the sloughed material angled to the sill of the opposite rib. The crew decided not to drive their tractor any further and walked out 550' to portal. Ground was past some anchors.
Electrical landing problem was discovered at 1 p.m. on 12/14/05. Problem was repaired and Alimak was put back in service at 3 p.m. on 12/16/05.
An unintentional roof fall occurred at teh 86+80 crosscut, entry #2 in the #2 return. The fall was 40' long, 20' wide & 7' thick.
When the hoist operator was clutching in, the hoist's safety limiting sensor recognized a clutch collar engaging problem and would not release the drum brake. Thirty minutes expired before maintenance could review the remedial action procedure with the hoist operator. No injuries or damage resulted from this incident.

An unintentional roof fall occurred approx. 100 feet inby ssn# 1187 in the #1 entry. The fall diamensions were approx. 80'L x 18'W x 4'H.
An unplanned roof fall occupped in 12-13-05 at 6 am at spad 12301 in the #5 & #6 entries at the #1 section of the old white night mines measuring 50' x 20' x 6', no persons were injured, travel was not affected, entilation was not affected. 8,000' tranactive section.
An unintentional roof fall occurred in the old 3D travelway at #30 xcut, 18+70' entry #5. Fall was 30' long by 20' wide by 8' high.
A roof fall occurred at 1:55 am on 12/12/05. Location was cross cut 148 entry 3 main south. Fall was in a four way intersection that had been mined October 15, 1996. Area was supported with 8' full resin bolts. Fall was 100' long x 20' wide x 20' high. No injury resulted however approximately 100' of belt line was lost. Plans are to clean and resupport area.
A fall of roof occurred on the 3rd S.E. Submains at crosscut 24 in the #3 Entry - supply road. The fall measured 20' x 20' x 12' high. No personnel, equipment or ventilation was affected.
Oakdale portal elevator was inoperative from 11:40 p.m. until 3:20 a.m. - No injuries - No entrapment - Malfunctioning door latch was repaired and elevator was operative at 3:20 a.m.
ROOF FALL D-MAINS, STATION #493 INTERSECTION OF #2 HEADING. 18' W X 40' LONG X 10' HIGH.
A 5 year load test was performed on the #2 portal passenger elevator. When the elevator was dropped one of the safety brakes was damaged. Replacement parts were delivered to the mine on 12/13/05. The elevator was repaired and a new 5 year test performed. Note: elevator was repaired, all tests completed and returned to service 12/16/05 at 4:00 pm.
Unintentional roof fall on 6N longwall section, #3 entry, conveyor/stage loader area. Fell approx. 35' long x 16' wide x 9' high.
A bearing on the govenor shaft on the Otis elevator failed causing the elevator to stop in mid trip. The men riding the elevator were stranded for approximately 10 minutes on the elevator. The elevator was removed from service and the bearing was replaced by Kone elevator maintenance personel on 12/11/05.
Mine experienced a rock fall measuring approximately 20' high x 20'-22' wide x 45' long.
A loose connection on a transformer caused a power failure to the plant air compressor which caused exhaust valves to close on the Pyro main baghouse which allowed hot gases to reach the dryer feed belts and started a fire. The Mobile Fire Dept was called and they put the fire out in less than an hour. Valves will be installed to prevent future like incidents
An unintentional fall of roof was discovered during the weekly exam one break inby spad #3163 on the Peters Branch Submains. The fall was approx. 20'x 20'x 4-6'.
A ROOF FALL WAS FOUND IN AN OUTBY AREA OF 9 MAIN ROOMS #1 BUTT. 20' WIDE X 200' LONG X UNDETERMINED HEIGHT. FROM #2 ENTRY THRU #5 ENTRY IN CROSSCUTS 16 + 60.
Employees were preparing the 23W 4840 Normat laydown excavation for ground support. As they were gathering materials, a wedge-shaped structure, 8'W x 8'H x 18'L failed. The area was supported by 5' split set bolts, 8' resin bolts and steel mesh. No injuries resulted from this incident. MSHA Helena office was notified by phone.
We were heating the hydraulic trailer for about five hours with a salamander heater. Employees left site about 2:30 pm with the heater heating the trailer. Got a call about 3:40 pm that fire started and fire department was called by the schoolhouse attendent that lives next door.
Motor generator set that generates direct current power for the production/man hoist would not synchronize. Problem appears to be temperature related. Item was reported to Dustin Creley who was on-site doing quarterly inspection.
THE MIRACLE RUN ELEVATOR BECAME INOPERATIVE DUE TO A WORN PLACE IN THE CONTROL WIRE IN THE TRAILING CABLE. NO PERSONS ON ELEVATOR. REPLACED THE TRAILING CABLE & ELEVATOR RETURNED TO SERVICE.
ROCK FALL 6 BREAKS OUTBY FACE #6 ENTRY 1 BREAK OUTBY SPAD #184. APPROX 20' X 20' X 4' THICK.
The #3 left pillar was being mined and the last run was being taken when a piece of rock measuring approximately 2' thick fell on the head of the miner, causing the miner to get stuck. Then a piece of rock approximately 4' thick x 20' wide fell on the miner causing it to be wedged between the roof and the bottom.
DISCOVERED UNPLANNED ROOF FALL IN OLD NO.1 ENTRY OFF #4 PANEL. ""OLD #3 BELT"", 1 CROSSCUT INBY SPAD 9189. AREA DOES NOT IMPEDE TRAVEL OR VENTILATION & HAS BEEN DANGERED OFF.
Three roof falls have been discovered. Fall #1 is at spad 4247 and is 20'X25' with an unknown thickness. Fall #2 is one break outby spad 4235 and is 20'X25' with an unknown thickness. Fall #3 is outby spad 4230 and is 20'X20' with an unknown thickness.
Roof fall in on Main dtake newtal hdg-just inby 3 head aber dev 1997 and bolted with 4' 3/4"" resin bolts. Thsi area hdg has a roof fall 20-30' inby this fall which has fallen in has a60. we pooted rib prior to fall since fall has installed 10' cable bolt in this area fall approx. 20' & 17' wide cover 150'. fall is 4' high.
A roof fall occured in the 6 North Panel, #1 entry, starting 10' outby the edge of #1 crosscut and ending 30' outby the edge of #1 crosscut. The fall measured approximately 20'X19'X6'. The area was supported with 6' tension rebar bolts.

Size of fall: L 30ft, W 19ft, H 10ft; type and length of roof support: resin 48"" - 72""; no timbers or cribs installed prior to fall. Strata: shale, sandstone, mixed. All entrances are breakered off. Area will not be rehabilitated, re-established return around fall.
A roof fall occured in the 1 east, crosscut #14, starting in the intersection of #9 entry & extending 20' into the pillar line between #9 & #10 entries. The fall measured approx. 40'x 19'x 6-8'. The area was supported with 6'tension rebar bolts.
Three additional roof falls have been discovered in old works. Fall #1 is one break inby spad 4268 and is 20'X40' with an unknown thickness. Fall #2 is in the three left crosscut at spad 4032 and is 20'X50' with an unknown thickness. Fall #3 is located in the #4 entry inby spad 4054 and is 20'X190' with an unknown thickness.
THE DESKINS B-SHAFT AUTOMATIC ELEVATOR WAS INOPERABLE DUE TO A SWITCH MALFUNCTION. THE READ-OUT FOR THIS ELEVATOR WAS SHOWING THAT THE ELEVATOR WAS ON THE SURFACE & ON THE BOTTOM AT THE SAME TIME.
The tail roller bearing of the #2 bottom project belt became hot while the belt was running and a fire started at the tailpiece. the fire ignited the belt fabric and this fire traveled 2400 feet from the tailpiece and burned the belt in half. People were evacuated from the mine. The fire extinguish the butby fire first and then extingished the fire at the tailpiece.
An unplanned stockpile fire occurred at this facility that was not extinguished within thirty minutes.
On the weekly examination of the mine the examiner found an unintentional roof fall in the return side of the 001 section (west mains) which has 2 other ways around it. Therefore it will not be cleaned up as it was timbered and dangered off. the fall was about 15' inby spad #3988 and measured about 50' long, 18' wide and 8 to 10 ft high. This old section is to be sealed.
WEST MAINS 26 BLOCK, 6 ENTRY, MMU 001. SUPPORT: 6 FT 3/4 INCH RESIN BOLTS, PANS, SCREEN, ACS JACKS, 12 FT CABLE BOLTS. 30 FT LONG X 25 FT WIDE X 7 FT HIGH. FALL DANGERED OFF, ADDITIONAL ROOF SUPPORT INSTALLED AROUND FALL. FALL CLEAN-UP PLAN DEVELOPED AND POSTED AT FALL SITE.
Had an unplanned inundation of water after continuous miner penetrated an old mine. No injuries and no damage to equipment.
A roof fall occurred in No. 2 Entry of TG-203. The fall occurred at the No. 16 x-cut. The intersection fell approximately 20 feet wide by 20 feet long and was approximately 10 feet high.
SLOPE HOIST INCIDENT - On Tuesday, 12/06/2005 at approximately 06:30 strands were discovered broken in the slope rope. A new slope rope was put in and the slope rope was operational at approximately 01:40 pm on Tuesday, 12/06/2005.

At 10:00 am on 12-6-05, a roof fall was found in an outby area of 9 mains, in #6 entry from station 2709 to station 2923. the roof fall was 20' wide x 80' long x unknown height.
An unintentional roof fall occurred in the face area of the E3-5B mine at approx. 9:30 pm. The fall measured approx. 30'long x 18'wide x 6'high & was slick-sided rock.
THE AUTOMATIC CONTROLS WERE DAMAGED WHEN A JACK CARRIER THAT WAS SUSPENDED FROM AN OVERHEAD HOIST SHIFTED ON THE WAY TO THE SURFACE.
A roof fall was discovered in the # 1 entry of A Mains panel; inby # 28 crosscut, extending to midway of # 29 crosscut. Fall is approximately 60' long; 18' wide and 7 feet high. The affected area was originally bolted with 5' hi-tech bolts and cribs added in the area.
A roof fall occured in the Main North, #5 entry, starting in the middle of the intersection of crosscut #11 and ending approximately 20? inby the edge of the pillar line between crosscut #11 and #12. The fall measured approximately 30'X18'X8'. The area was supported with 6' tension rebar bolts.
THE ELEVATOR AT THE #8 PORTAL WAS INOPERABLE FOR A PERIOD EXCEEDING THIRTY MINUTES DUE TO THE BRAKES NOT OPERATING PROPERLY, THE BRAKES WERE REPAIRED AND CLEANED AND THE ELEVATOR WAS PLACED BACK INTO OPERATION.
A roof fall has occurred in an intersection two breaks inby spad # 4670 in the #3 entry. The fall measured 20' X 20' X 7' in thickness. This was not on an active working section. Additional roof support was placed in the area after the fall.
Right mains across from spad 1249, #8 entry in intersection. 8'high x 40'long x 16'wide. Mined Oct.2005. Bolted with 5'resin bolts & spot bolted with 8'bolts.

An unintentional roof fall was found on the 2 left mains section in the #8 entry 2 block inby spad staiton 3997. the fall measured 18' w x 7' high x 19'long.
On 2rt. #1 Panel x7-8 fell in at 6 bldg-apporx. 6 1/2-7' high by 17' wide top was shale and laminated div on 12-2-05, pouted off and day will not be cleanely area bolter with 6'-3/5"" fully grouted resin bolts-cover of 3500 fall was in area of bolt top ans posted off a d.o. before it fell.
A piece of rock fell in break, one break above 42 switch end. The rock fell to the edge of track. The rock was approximately 12"" sandstone, 24"" slate and measured approximately 7.5' wide x 10' long x 36"" thick. The rock fall pulled 2 roof bolts.
Had an unintentional roof fall 5 breaks inby #25 head drive.
The fire started in a reclamation field in Area D of the mine. The grass in the area was dry and approximatly 12 inches tall. No work was being performed in this area at the time of the fire. The fire covered an area 5.48 acres. There were no structures in the area and the fire presented no danger to mine employees. It is unknown at this time how this fire started.
Grass fire in non-active area of mine. No injuries. No equipment damage. Fire allowed to burn out due to wooded area and terrain. Kept fire watch. Fire started from smoldering wood from previous fire (12/1/05 - fire did not last 30 min.).
A roof fall occured on the 006 section in 6 right over to intersection of 7 right. The rock fall measured 24 foot long, 18 foot wide and 7 foot thick.
A roof fall occurred on the 3 West A belt at crosscut 54 in entry #4. The cause of the fall was weak laminated shale. The fall covered the 3 West A belt.

AT THE TIME OF THIS REPORT WE BELIEVE A LARGE VOLUME OF ROCK HAS BURST FROM ABOVE THE PERIMETER HIGH WALL. THE BURST MATERIAL & RESULTANT SLIDE EXTENDS INTO THE TREE LINE ABOVE THE QUARRY RIM. NO INJURIES HAVE BEEN EXPERIENCED AND NO EQUIPMENT DAMAGED AS A RESULT OF THIS INCIDENT. FURTHER INFORMATION WILL BE AVAILABLE ON INSPECTION AT 12-2-2005
Roof fall in the main north track entry between 20 and 21 crosscuts inby the #2 portal. Roof fall is @ 50 ft long, 17 ft wirde and @ 9 ft high. Roof material is clay/shale. Area was developed in august 1993 with a continuous miner and supported with 8 ft point anchor roof bolts.

12'L x 16'W x 5'H rock fall on ellis mains between #1 and #2 entry near spad #578.
An unplanned roof fall occurred in the #5 entry of 1 left. Fall was approximately 18' x 60' x 4' plus high. Area was bolted with 48"" fully grouted resin bolts with 72"" tension rebar bolts spotted throughout this area. Did not affect any equipment or ventilation. Will not be cleaned up.
60'L x 18'W x 5'H rock fall on ellis mains in #1 entry near spad no. 519.
A roof fall occured in the 1East, crosscut #12, starting in the middle of the intersection of #8 entry and ending in the middle of the intersection of the #9 entry. The fall measured approximately 65'x18'x6'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occurred on the 7-left longwall headgate/belt entry. The tip of #1 shield was at 55+35 and the fall extended outby for 70' to 54+65. The height of the fall was from 7 to 9 feet; this was above the anchorage horizon of the 6' bolts used in the initial mining.
The victim was caught between 10SC shuttle car and an 818 Long Airdox ram car resulting in severe lower leg injuries and death.
While in the process of installing permanent roof support in the No. 1 Entry, the roof started to fall and the employee ran inby the ATRS System. The roof fell in this area causing crushing injuries to the employee.
Employee was mining a cut of coal when a horseback fell out causing fatal injury.
While installing structural steel doors over coal bin EE went around safety rail without using fall protection on the door which was still attached to the crane to try to move the door & close a gap between the doors. The door shifted causing a support I-beam to move shifting the doors which caused him to fall. MSHA was notified 12/27/05 that he had expired from his injuries.
The Euclid R-Haul truck Comp. No. 671 was hauling a load of stemming material to the drill bench in the Wadesville Pit. The truck went over an embankment, upset, coming to rest on the driver's side causing fatal blunt force trauma to the driver located inside the cab. The cause of this accident is yet to be determined pending further investigation.
Employee went to crusher area to investigate noise coming from the six roll crusher. He proceeded to control panel, switched #14, 15 & 16 from auto to hand (manual) then hit stop buttons. He went back to crusher & removed inspection plate, reached in after spotting rock on top of bottom rollers, catching fingers in bottom roller.
Employee was cutting away section of chute work when it broke loose it caught his glove and pulled his hand in pinning his finger between chute and deck, causing amputation of the tip of left middle finger.
EE IS A SURVEY TECH. & WAS ASSISTING MOVING EMPTY PALLETS, CLEANING A LAY DOWN AREA. HE FELT A STRANGE SENSTAION IN HIS LOWER RIGHT ABDOMEN. AFTER SOME TIME & SEVERAL Dr. VISITS, HE WAS DIAGNOSED WITH A MINOR RUPTURE THAT HE MAY NEED SURGERY FOR. NO TIME MISSED FROM WORK.
Employee was helping move dragline trail cable & was pulling on rope holding cable to attach to hilift when he felt a pain/tear in right side groin. Stated he already had a hernia condition before this injury. This accident was not reportable until surgery was required by a doctor on a visit on 1/3/06. Surgery scheduled for 1/20/06, will result in loss time.
Ee sustained injury to his left big te when an airlock door came in the deck of personnel carrier he was riding in EE sustained crushing injury to his left big toe. EE big toe was amputated to the first joint.
Employee was moving stoper drill when he felt pain in his upper abdomen.
Employee stated he was attempting to make corrections in the Pemco 1500 KVA section power box. While he was working in the section power box he came in contact with an electrical current, resulting in an electrical burn to his right hand palm and elbow.

MECHANIC WAS CLIMBING ON SERVICE TRUCK, FELT POP IN FINGER.
Install pedistal in beam for hi wall miner and strained his back was not reported until l 5-05.
EMPLOYEE WAS ROOF BOLTING WHEN HIS STEEL GOT STUCK IN THE ROOF. EMPLOYEE STARTED TO DRILL A HOLE RIGHT BESIDE THE OTHER ONE AND HAD HIS HAND ON THE DRILL HEAD. VIBRATION WHILE DRILLING THE OTHER HOLE CAUSED THE STEEL THAT WAS STUCK IN THE ROOF TO COME OUT STRIKING EMPLOYEE ON LITTLE PINKY ON LEFT HAND, CUTTING IT REQUIRING STITCHES.
Employee moved the lockout sign so he could mount the water truck. As he ascended the ladder, the sign got in the way and he missed the step. He fell to the ground resulting in a fracture to his right wrist.
Employee was hanging a continuous miner cable and felt a sharp , stinging pain in the left leg that moved to the lower back the next day.
LOCATED ON THE TAILGATE #2 DEVELOPMENT (GATE) SECTION, THE EMPLOYEE WAS ATTEMPTING TO REMOVE A BIT INSERT FROM THE VOEST-ALPINE MINER CUTTER HEAD. AFTER STRIKING THE INSERT WITH A SLEDGE HAMMER, A SLIVER OF METAL FROM THE INSERT WENT THROUGH HIS GLOVE AND BECAME IMBEDDED IN HIS RIGHT MIDDLE FINGER. BEGAN MISSING WORK ON 12/31/05.
WAS WALKING ALONG CAT WALK ON HITACHI 700 TRACK HOE WHEN EE SLIPPED AND FELL OFF OF THE CATWALK. HE LANDED ON HIS BACK ON A BOULDER
A PIECE OF DRILL STEEL WAS STUCK IN THE ROOF. THE OPERATOR WAS TRYING TO REMOVE IT WHEN HE PLACED HIS RIGHT INDEX FINGER AND THUMB AROUND THE STEEL NEAR THE ROOF AND APPLIED PRESSURE ON STEEL CAUSING PUNCTURE TO INDEX FINGER AND THUMB.
Injured was unloading and cleaning Alimak work deck when he fell off of the elevated nest platform onto the muckpile 6 to 8 feet below resulting in a laceration to the face from the eye to the chin.
Employee was walking along the Longwall face and stepped on a relay bar which was under water and slipped twisting his knee. Note the time between accident and reporting, employee first day of restricted duty was Feb. 24th 2006
The alleged accident resulted from employee manually lifting a partially filled 55 gallon onto the rear of a company service truck. In doing so, EE states that he pulled a muscle in the calf portion of his leg, which may have contributed to blood clot(s) in his leg. A portion of this blood clot(s) traveled to EE's lung causing him to be hospitalized.
Injured was tramming toward stacker when the loader he was operating hit a hole and jarred the operator.
OPERATOR WAS DISMOUNTING DOZER AND STEPPED ONTO UNEVEN GROUND, THUS CAUSING HIS RIGHT ANKLE TO SPRAIN.

EMPLOYEE WAS PULLING WATER HOSE FOR MINER WHEN HE SLIPPED AND FELL STRIKING RIGHT ELBOW ON 4"" HARD WATERLINE CAUSING BRUISE. EMPLOYEE MISSED FIRST DAY ON 1/2/06.
Employee was crawling over the front bridge, in E-2 Section. When he got over the bridge, his left foot was on the mine floor and he twisted his body to get the other foot over. He twisted his left knee.
Employee was in quarry/pit standing on a marble block on edge of 5'-6' ditch. The block gave way and employee fell into ditch on another piece of marble. Complained of pain in left foot, left hip, right leg, right wrist and back. Employee was not able to move. Ambulance took to hospital.
Employee was driving a pickup, bounced up and came down with his elbow on his lunch box that was sitting on the seat, pulling his shoulder. Medical treatment, was sought on 12-29-2005, he was released back to full duty. His shoulder continued to bother him and he underwent surgery on 3-23-06, for a torn rotator cuff.
1W # 3 Take-Up - Employee was cleaning 1W # 2 tail. hit head, causing a burning sensation in middle of back.
AFTER LOADING A SHOT AT THE FACE THE EMPLOYEE WAS CLEANING UP BOXES AND DEBRIS AND PUTTING IT ON THE TRUCK. HE WALKED BY THE REAR RIGHT TIRE OF THE TRUCK AND IT BLEW OUT JUST AS HE WALKED BY.
INJURED WAS SERVICING FEEDER WHEN HE TRIPPED ON CABLE.
Employee was struck on the left ring finger by the bolt driver after installing a ground support bolt. The finger was fractured.
EE operating a Volco L150C front-end loader scooped up a bucket of ore, drove to hopper. As he drove a 1' x 3' chunk of ore fell out of bucket, EE drove over the chunk causing the EE to bounce in cab of front-end loader, causing the neck injury.
EE STATES HE SLIPPED ON ICY CATWALK (NEVER FELL) AND STRAINED HIS BACK BACK IN DECEMBER (NOT SURE OF DATE)BACK IS NOW BOTHERING HIM (REPORTED TO COMPANY 2/2/06) HE WAS WALKING CATWALK INSPECTION BELT LINE. HAD THE ACCIDENT BEEN REPORTED WHEN IT OCCURRED THIS FORM WOULD HAVE BEEN FILED THEN.
Drive belt on conveyor motor was loose & slipping. Belt was shut down for repairs. After belts were tightened plant operator notified affected EEs in area that he was going to start belt back up. Repeated as he walked back to MCC. IE noticed some rock in & around belt pulley & informed operator but he didn't hear. As IE reached in to remove rocks, belt started up.
A rock fell out of the top striking the employee on the right hand and third fingers.
Rock fell and pushed hard hat into right eye brow cutting his face.
Hopper beneath S3-L had filled with rock dust when the shaker's bottom conveyor failed apparently due to the failure of the convyor's v-belts. The employee entered the hopper via the bottom or tail end to shovel out the plugged hopper. The material shifted inside causing the conveyor to move pinching the employee between spilled material and a hopper cross-brace.
Employee was filling the antifreeze tank, when he pulled the hose from the tank he got antifreeze in his eyes.

Airing tire up, sidewall blew out. Tire was still mounted on F450. Tire was inside; outside tire had been removed. Tire only had approximately 400 miles.
Employee step out of cockpit of LHD. When he dismounted he stepped on a chunk of salt causing him to twist his left knee. Employee felt slight pain in knee but went back to work. Employee came to work on 3/20/06 with Doctor's excuse putting him off work and stating that he needed knee surgery.
EMPLOYEE STATED THAT HE WAS CROSSING THE CROSS-OVER OUTSIDE WHEN HE SLIPPED AND FELL, STRIKING HIS RIGHT SHOULDER.
Employee stepped into a hole and twisted his ankle.
EE STATED THAT WHILE HAULING COAL HE HIT A HOLE IN THE ROAD, CAUSING HIS SEAT TO BOTTOM OUT, HURTING HIS BACK.
THE WORKER WAS STANDING ON AN EXCAVATOR TRACK WHEN HIS FEET WENT OUT FROM UNDER HIM AND HE SAT ON HIS TAIL BONE. THE RESULT IS A BRUISED TAIL BONE.
On 3/27/06 the employee notified the mine operator of lost time due to an injury sustained on 12/27/05. The employee attempted to lift a crusher inspection door and strained his right arm, torn tendon.
Employee exited the loader cab and tripped over the fuel cap on the deck outside the cab and struck his knee on the rail/deck. Employee was taken to the hospital for x-rays and has not returned to work as of this date. Carelessness of the employee contributed to the injury.
Rock fell from highwall & hit employee on shoulder while on auger.
Ee was exting bobcat skidsteer loader and didn't notice support beam for granular bulk truck load out canopy. He hit head on support beam and wrenched neck. He was examined with a negative x ray, but a smucle relaxer was preseribed and 1 shift of work was missed. Physician ordered 1 shift to be missed-that he not return that next evening.
While cleaning up around a ditch pump, EE turned on a water hose and while attempted to unravel it she slipped and fell on both knees. Did not seek treatment until 3/29/2006. The claim was protested based on time of medical treatment but later was determined on May 16, 2006 to be work related.
Employee was getting out of the fork truck when his foot got stuck between the seat and side of the cab, causing him to twist his right knee.
Employee was walking up a hill when he slipped and fell, spraining his left hand. Ground was snow covered and slick. Employee was attempting to light a pile of brush. He notified me 12-30-05.
An employee was descending down a front end loader and at the bottom step he turned while stepping to ground, lost his balance, twisted his knee and fell to ground.

Employee injured back while driving slope belt drive bull gear onto shaft with sledge hammer. Note: This accident was not ruled compensable until 3/7/06, received notification from Brick Street on 3/9/06. Originally beleived accident was not work related since first day missed was 1/26/06.
Employee was adjusting seat in Euclid haul truck, felt burning sensation in lower back. Went to doctor on 12/29, low back strain.

He was climbing up the side of the dozer and lost his footing. He fell backwards onto the ground.
Employee was climbing down from east crane trolley when he felt a sharp pain in back of left knee. Experienced discomfort in knee but had no medical attention until 2-25-2006 when pain got worse. (Began missing work 2-27-2006).
Employee slipped, fell and landed on right shoulder began missing work 2/22/06.
Employee was pulling cable away from miner while moving. after lifting cable, foot slipped causing employee to fall against miner.
EE SLIPPED ON SLATE AND COAL WHILE FILLING WATER TANK ON THE MAINTENANCE VEHICLE FOR FILLING BATTERIES. EE FELL DOWN UNDER THE VEHICLE AND HURT HIS RIGHT HIP AND RIGHT SHOULDER - LOWER BACK PAIN. INITIAL X-RAYS NEG. 1ST DAY MISSED WAS 1/6/06 (MINE WAS SHUT DOWN 1 WEEK AT CHRISTMAS). X-RAY & MRI BY ORTHO. SPECIALIST. DIAG. VERTEBRA PROBLEM, SURGICAL REPAIR REQUIRED.
Employee pulled a muscle in his lower back while performing his regular duties. The exact cause is unknown, however, he had been sweeping,shoveling, operating a back hoe, and around 3:30 p.m. he helped another employee lift a 1/4"" plate of steel from the saw horses. Later that evening, when he was exiting his truck at home his back was hurting.

HE WAS TRAMMING BUGGY AND HIT A HOLE CAUSING HIM TO BOUNCE UP HITTING HIS HEAD AGAINST CANOPY STRAINING HIS NECK AND BACK. HE DID NOT GO OFF WORK UNTIL 12/29/05.
Employee stated that while attempting to open tailgate, he was using the bullpin out of the bumper to try to pry the tailgate open. The pin slipped and mashed his thumb between the pin and bed.
Member tripped over hoses and struck back against a scoop.
While loading a truck with rock (boulders) one slip around and trapped the left leg. at the back.
While putting away lube hose on the lube truck, the employee bumped up against the rib and touched a shuttle car cable. There was an open spot on the cable and the employee received an electrical shock in his buttock and down his left leg into his foot. He was taken to receive medical attention and released He went back to the Emergency room and was given one day off.
EMPLOYEE WAS ATTEMPTING TO BEND A 6' ROOF BOLT AND HE FELT A POP IN HIS LEFT SHOULDER. AS HE WAS STRAIGHTENING THE BOLT, HE FELT ANOTHER POP IN HIS SHOULDER ALONG WITH A TINGLING SENSATION IN HIS LEFT ARM. HE DISLOCATED HIS LEFT SHOULDER AND IT POPPED BACK IN PLACE.
Employee was lifting a motor and setting it down as he felt pain and heared a loud pop in his left arm.
The victim twisted his right knee while helping to load a section of high voltalge cable. He finished the shift but was unable to return on his next shift.
Employee was checking fluid level on loader. The fitting came loose and some fluid passed his safety glasses and into his eye.
EE was doing routine maintenance and the wind blew debris into his right eye (conjunctivitis).

While attempting to climb down the ladder of a drill, EE stepped off the bottom step onto uneven ground and turned his ankle causing a fracture. Restricted work duty no lta.
Employee was tramming miner when he slipped & fell catching himself with his right hand causing a strain to his right shoulder. Employee did not miss any work due to this injury until 2/8/06 - Had an MRI on 2/8/06.

A crew of 4 men were installing a belt, using a forklift and boom truck. Two employees were watching. One stationed at the tail pulley; one between the tail pulley & boom truck. The Forklift was on the opposite side of the conveyor pulling the belt. As the belt went around the pulley the employee reached in. The belt jumped and pulled his hand into the tail pulley.
THE EMPLOYEE WAS PULLING THE STRAP TO PLACE IT IN THE CONVEYOR. HE FELT SUDDEN PAIN IN HIS LOWER BACK UP TO THE WAIST.
Belt splilled. When attempting to apply pressure with foot felt pop and pain in right side of foot.
During repairs to feeder/tailpiece combo employee became entangled with tail roller, suffering serious injury to his right leg.
Employee lifted a stone off a skid, turned to set it on the rollers and felt a sharp pain in his back. He did not seek medical treatment until 12/22/05. Acute back strain. Employee unable to work.
Employee was exiting a 936F wheel loader, while climbing down the steps he slipped and twisted his left ankle.
Employee was cleaning frozen material off of a catwalk when a piece of material chipped off and entered his eye.
The employee stated he was pulling nip cable at the shop and tripped and fell hitting his left hand.
MEMBER WAS CUTTING A HOLE FOR A MOTHER DRIVE WHEN A PIECE OF ROCK FELL & HIT EMPLOYEE ON HIS HEAD, ON TOP OF HIS HARD HAT.
Employee was carrying pipe to pump concrete thru and jammed his neck on a truss bolt installed along the supply road. Employee didn't go to the doctor until Jan. 13, 2006. He begin missing work on Jan.14, 2006
Employee was geting out of repairman's buggy, slipped and fell in mud.
Employee was handling 8' ice covered timbers. Timber slid causing him to hyper extend left arm. Worked rest of shift and was then taken to doctor where he was diagnosed with possible torn rotator cuff.
Employee apparently pulled a muscle in his lower back when he slipped on ice at the fuel farm. He had exited the cargo area of the fuel truck and when he stepped on the concrete pad, his foot slipped on the ice jerking his lower back.
Employee was reaching down to pick up a curtain when he was struck on the left leg by a draw rock.
Employee was lifting a guard on a conveyor belt take-up, and felt a pop in his lower back (lower back & leg strain). Employee did not seek medical treatment at the time. His injury was later diagnosed as a disc herniation, which eventually required surgery.
The employee was loading his truck while using 843-057 loader. The loader bounced over a hole in the ground causing the cab of the loader to jostle the oeprator causing pain in the low back.
EMPLOYEE WAS PICKING UP A BUNDLE OF 8 FT ROOF BOLTS TO PLACE IN THE BUCKET OF A SCOOP. HE COMPLAINED OF BACK PAIN. NOTE: EMPLOYEE FIRST MISSED WORK DUE TO THIS INCIDENT ON 12/27/05.
Injured party had picked up the 4"" water line at the unit tailpiece to put on a fire drop when he felt a pain in his lower back.
Employee got off manbus to go to the wall, stepped on a rock and turned his ankle.
Employee stated that he was trailing empties into the mine on #104 motor when he hit his head on the top at approximately 22+50. After further investigation on 12/29/05 it was determined that this incident would be classified as a lost time incident.
THE CONTRIBUTING CONDITIONS OF THIS ACCIDENT WERE: (1) AN EMPTY MACHINE DID NOT YIELD RIGHT OF WAY TO A LOADED MACHINE, (2) THE EMPTY MACHINE WAS RUNNING TOO FAST FOR THE CONDITIONS.

Employee doesn't know what happened. He had no pain during work or when he left work. He started feeling pain about 7 pm at home.
THE INJURED WAS A PASSENGER IN A COVERED MANBUS WHEN THE DRIVER OF THE MANBUS ATTEMPTED TO STOP & THE MANBUS SLID INTO A PARKED MANBUS, CAUSING EE TO INJURE HIS NECK. THIS EE BEGAN MISSING WORK ON 12/28/05.
Employee alleges moving a space heater and straining his lower back on 12/19/05. EE continued to work and recently sought medical attention.

EE was operating the continuous miner (Alpine) through lunch on the 10-A section. While taking a short break a piece of roof coal fell and struck him on the hard hat. He looked up to check the roof conditions again and another piece had already fallen. A 5""x10"" piece of coal struck him in the face causing a laceration to his upper lip and chipping a tooth. Cut required 5 sutures.
Employee stepped down from steel platform he was working on and twisted his left ankle, tearing ligaments.
Operator assisting mechanic to drive ripper shank out of socket on tool bar. Metal shard struck left inner thigh becoming imbedded in flesh. Cold morning - about 10 degrees.
Worker was using hand grinder to shape piece of metal. After 15 or 20 minutes of grinding a small piece entered worker's right eye causing irritation. The piece was secured to work bench. The worker is experienced at using such equipment and was wearing proper eye protection.
Trying to turn on light in the shop and tripped over a platform scale in the dark and fell.
Employee was operating the #4 Fairchild Bridge, the crawler unit turned without his knowledge and when he started to tram, the machine trammed onto his foot.

The timber had been cut to length & 2 co-workers lowered 1 end to the ground from the scoop & EE & another co-worker were holding other end of timber. W/out telling EE, other co-worker lowered the timber to rest on his knee, shifting the weight to EE who experienced pain in lower back.
WHILE INSTALLING PRIMARY ROOF SUPPORT IN THE FACE OF #1 WEST, THE EMPLOYEE WAS STRUCK IN THE CALF OF HIS LEFT LOWER LEG BY DRAW ROCK. THE ROCK 2' X 3' X 3"" FELL BETWEEN THE PREVIOUS TWO ROWS OF ROOF BOLTS, STRUCK THE OPERATOR'S CANOPY, THEN STRUCK THE OPERATOR. AN AIR SPLINT WAS APPLIED AS A PRECAUTION. HE WAS TAKEN TO THE SURFACE, THEN TRANSPORTED TO THE LOCAL HOSPITAL FOR X-RAYS.

PERSON WAS MANUALLY ADJUSTING FORKS ON OUTSIDE FORKLIFT AND FELT PAIN IN RIGHT LOWER BACK. PERSON WAS TREATED BY CHIROPRACTOR FOR THIS PAIN ON 1/23/06. Misc Mail--Started missing work 3/6/06.
Injured was in the process of loading timbers in a scoop. One of the timbers was not completly spirt. While attempting to seperate the timber he fell backwards on his rt. side. A hammer he had in his light pouch pressed into his ribs.
Employee was checking voltages and fuses on an electrical switch at the main plant. While checking the fuses, he had the meter setting on Ohms. When he started checking voltage, he forgot to change the meter's setting. When he checked the voltage, the phase to phase contact caused an electrical flash when the meter's probes came in contact with the current.

Employee was drilling a hole with a bolter wrench when the bolter wrench hit him on the top of the hand.
Ee was fueling a dozer when his feet slipped and he fell off the dozer. His left ankle was severely sprained.
Employee was climbing 4-foot step ladder to gain access to D10 dozer in order to fuel it. When the employee reached the 3rd step, the ladder started to wobble and the employee lost his balance and fell. This resulted in a compound fracture of the right lower leg.
Employee stated he was turning around in the deck of his shuttle car when continuious miner backed up bumping car and pushing him to back. Stated he bumped his back against roof (height approx. 4 1/2 ft) and did not want ambulance or medical care. Rode out side on 3 wheeler.

Riding slope mantrip car down the slope. When the car reached the slope bottom and came to a complete stop, he attempted to unboard the passenger compartment. He swung himself from the seating compartment. His foot somehow went in under the car, his foot catching on the track rail and twisting his knee. Employee was operating overburden belt fed by the backhoe. A large chunk of overburden fell off the top of the belt hitting the employee on the lower left side of the back.
EE stated that he started to turn around, and he felt pain in his left knee. This ee stated that he has had cartilage removed from both knees when he was in grade school. He is schedule to have an mri on 12-16-05.

Employee was standing on excavator steps while fueling excavator. One foot slipped from excavator step; employee pitched forward and stopped fall with one hand by placing same on excavator deck. The impact stress of stopping his fall resulted in rotator cuff injury.
While plate cleaning the operator had to move the plates manually by hand. Usually mill operations uses a plate bar to assist moving the plates, but there is a wash water mounting bracket in the way so the operator had to use his hand to move the plate and one side moved faster than the other. The plate fell off the rail and pinched the employee's right hand ring finger.
Trying to put air filter cap back on from the right tire, he slipped off tire to the ground landing on right hip.

Employee was performing start-up service on high pressure hydraulic pump for longwall. He adjusted packing to correct small leak and asked section mechanic to restart pump. On start up, high pressure hose & ball valve came apart striking employee under chin. Knocked out 3 teeth, broke mandible & damaged tissue in inner left ear.
MECHANIC WAS PREPARING TO WELD PIN LOCK ON BOOM OF EXCAVATOR WHEN WELDER MISFIRED. EMPLOYEE TURNED TO GET AWAY FROM SMOKE & HE TRIPPED OVER A GASOLINE CAN & FELL TO THE GROUND ON HIS LEFT SHOULDER.
Fire was started by one employee and inhaled by another.
Employee was starting a 992D loader. When he went to dismount the loader, he slipped down the steps. It was snowing and the step were slick.
Employee was getting an oxygen bottle when he dropped it on his right foot. It caused a bruise and contusion to his foot. He started missing the next day.
Employee stepped onto the step of the rock truck and the strap holding the step broken. Employee fell on his side onto the frozen ground

Employee was pulling structure when a rail slipped from his hand and hit the top of his left foot causig a contusion.
Employee was assisting two fellow employees install rib mesh along the walkway side of conveyor belt entry. As he pushed the wire mash against the rib, a loose piece of rib fell pushing the mesh toward the ee. EE attempted to turn and move away, but his arm/elbow caught on the mesh, resulting in shoulder strain.
Employee was walking on catwalk slipped and strained shoulder.
The operator of a 777Cat Haul Truck hit a soft spot in the haul road at an excessive speed, this resulted in a fracture to the operators right wrist, and a laceration to his chin. Note: The operator was wearing his seatbelt, also, the operator had been through the soft spot 4 times prior to the accident, and was contacted approx 1 minute prior to the accident about the soft spot.
Employee was stepping across a piece of ice and his right foot flew out from under him causing him to fall. His left leg was bent under him, spraining his left knee.
EMPLOYEE WAS USING A SLIDE HAMMER TO REMOVE A TRAM SHAFT ON THE F-12 LONGWALL SHEARER AND PINCHED HIS LEFT RING FINGER BETWEEN THE SLIDE AND SLIDE STOP CAUSING A FRACTURE AND LACERATION TO HIS FINGER, REQUIRING 10 SUTURES TO CLOSE.
Employee was striking a link pin with a ball peen hammer. Small piece of metal struck and embedded in left wrist.
Employee was stepping into the face conveyor, to attach a cable when he struck the front ok his knee on it.
Construction stopping on No.2 unit. He was plastering the brattice. one the sole of his work boot was damp plaster. When he stepped through the man door, his foot slipped due to the wet plaster resulting in twisting of his left knee. He continued to work all scheduled shifts unit 1-4-06. became a lost time event on that date.
Injured stated that on 12-10-05 he was kneeling down to shovel some material when as he knelt down he struck his knee on a rock. He had no lost time until 12-14-05.
Employee was operating payloader. He parked the loader to get off. While coming down the ladder the employee missed the last step falling to the ground and injuring his right wrist.
Injured was hooking up miner cable to 3-wheeler with rope hanger. Said foot hit tram pedal before he could get fingers out of rope hanger.
EE was shoveling take-up out mud & water his foot got stuck in the mud he could not pull it out, he asked another man to help him pull it out, when it was freed up he walked out of mine but once outside he said he could not walk.
Employees were using a come-along to pull a new impact slide pan into a belt tail piece. They were aligning the bolt holes up and as the pan was pulled foward, his finger was caught between two pieces of metal/ Avulsion/ pinched off finger tip.
REPORTED CUMULATIVE TRAUMA TO LOWER BACK ON 12-9-05. COMPANY INVESTIGATION COMPLETED 12-15-05, CLAIMED INJURY IS DETERMINED TO BE WORK RELATED.
Calibrating #10 clinker fow. Test chain (74 lbs) was being placed on storage hook at #7-8 fow area. The test chain got stuck on glove. He let go of chain thinking it was hooked. Chain fell from hook twisting & pulling left arm down & back. Surgery May 22, 2006.

Employee pulled his knee(L) while working on a trainer in a dolly. He didn't start missing work until 3/6/2006 when his knee was operated on.
Employee was working in the ball mill removing plates from partition. Employee fell through exposed hatch in floor, falling 10 feet to concrete floor below.
EMPLOYEE WAS STANDING ON TOP OF ROTARY BREAKER THAT WAS BEING DISMANTLED (APPROX. 11 FT. ABOVE THE GROUND). HE WAS CUTTING SCRAP METAL USING WELDING TORCH. HE STEPPED ON THE PIECE JUST CUT AND IT BROKE, CAUSING HIM TO FALL TO THE GROUND (BACK STRAIN).

EMPLOYEE WAS ENGAGED IN HIS REGULAR JOB STACKING 50 LB BAGS ON PALLET. HE PLACED A BAG ON PALLET, FELT A POP IN HIS BACK AND SOME PAIN. HE DID NOT THINK IT WAS ANYTHING TO WORRY ABOUT AND FINISHED HIS SHIFT. HE REPORTED THE INCIDENT ABOUT A HALF-HOUR AFTER IT OCCURRED BUT DID NOT DECIDE TO SEE A DOCTOR UNTIL MONDAY, DEC 12TH.
While repairing a concrete jaw crusher,lifted an object and felt an inguinal groin strain.
Injured was changing out a set of batteries on his hauler. As he was lowering the lid on the batteries 6 cells grounded out. This caused battery acid and other debris to strike the injured in the face.
While operating a haul truck, the operator noticed smoke coming from the battery box. He stopped the truck, went over to the box, raised the lid and the battery exploded. A piece of the battery hit him in the nose and caused a laceration. Also, acid splashed in his left eye.
Picking up drill he hurt his back.
Employee was helping unload a metal tool box from the compartment of a track jeep when he experienced pain in rt. side of low back and rt hip.
Filling desiel fuel tanks at east dam; slipped and fell on ice.
While working to remove a sheave wheel from its spindle on the 1st floor of the Prep Plant, the wheel broke free and caused the employee to quickly move out of the way. Employee struck the edge of his left foot against the concrete base of an electric motor. He sustained a fracture to the 5th metatarsal of his left foot.
Employee was helping move the belt for West 3 section. As he was raising a belt rail to attach a spreader bar, he felt a pain in his lower back. He was diagnosed with a extradural mass at L2-L3 disc and will require surgery.

A clasp was hooked to the barrel edge and as it was moved from the pickup truck, the clasp released and the barrel dropped about two feet onto employees rt foot. Employee had a hairline fracture on top of foot; no medical treatment, just time off for swelling to go down.
INADEQUATELY SECURED EQUIPMENT ROLLED OUT OF STORAGE TRACK INTO PATH OF SHOVING MOVEMENT. EMPLOYEE JUMPED FROM THE RAIL CAR HE WAS RIDING PRIOR TO THE COLLISION.
Employee was using a hog burner to clear a frozen chunk from a chute when the chunk broke free and pinned the employees hand against the rail.
It was snowing heavily and employee was walking down a ramp when he slipped and fell causing a fracture to his leg.
An employee with 18 months experience was hauling shot rock to crusher. He came around right handed curve got off left shoulder, over corrected causing truck to turn over. Employee's left elbow cut and scratched. Employee was transported to hospital where he received 7 stitches to left arm. X-rays revealed bruised left shoulder and arm.
Mantrip was bringing crew outside. When mantrip went around a turn a jack slid or fell over striking employee's lower right leg.
Miner was dismounting from JCB Miller forklift. His left foot was on the ground and his right foot was on the step. When he moved his right foot to step to the ground his left ankle rolled over causing a sprain of the left ankle.

While pulling maintenance on 2"" conveyor tail pulley, the icy belt that employee was standing on rolled with him pinning his right ankle between pulley & belt.
This is not a work-related injury, not covered by workers' comp but it did happen in the employee parking lot before Employee and co-worker clocked in. Employee was walking in the employee parking lot and was struck by co-workers personal vehicle. Right arm and right leg were bruised and strained. Employee missed work 8 & 9 Dec and returned 12 Dec to regular duties.
The employee unhooked a load that was placed by a crane, a coworker did not maintain control of the headache ball allowing it to swing into the injured employee - striking him in the head.

When he returned, an employee noticed he ran into a boulder placed in front of our scale house and went out to check on him. He complained that his left arm was unresponsive. He was taken to local ER.

Employee was repairing brattice that had been damaged by a piece of equipment. Several blocks had been knocked out but some were still in place at top of brattice. As employee started replacing blocks a block above him fell striking his left thumb.

Injured was clearing a return roller on an outside plant belt that was blocked with frozen material. He lost his balance and his forearm was momentarily caught between the belt and idler roller causing a severe contusion to his arm. Injured shut the belt down immediately.
Ee was removing a frozen valve 3"" to thaw it out once all bolts were removed the valve was stuck in place. He pulled on valve when it broke loose it. He did not have a good grip on it and it fell on his wrist the valve weighed approx. 40 lb.
PERSON WAS HELPING TO LOAD VULCANIZING TOOLS ONTO CAR TO SEND OUTSIDE. WHEN PERSON BENT OVER TO LEFT COOKER PLATES, HE FELT PAIN IN LOWER BACK.

Ground was frozen, snow had an accumulation of 2 inches. EE was exiting his vehicle at the N.E. holding pond to start a pump. He had traveled approx. 10 ft and slipped. He broke his left fibula and damaged ligaments in his left ankle that requires surgery to insert 2 pins to stablize his ankle.
Getting down off of loader his foot slipped off of bottom step. Foot landed on a rock.

THE OCB OPENED BUT FAILED TO EXTINGUSH THE ARC ON TWO PHASES WHICH CAUSED AN EXPLOSION.
The injured employee was assisting in pulling drisco pipe. The employee was between the drisco and a berm when the pipe came into contact with his ankle. The employee was taken to a doctor where x-rays revealed a broken ankle.

Employee working on forklift tire. Tire blew out knocking employee down causing facial lacerations and bruised ribs.
U/g electrician was traveling via Kubota tractor from the electricians shop on the 920 main to the top of 45 decline to access the M165 level in 200S to repair a damaged switch box. He was traveling north and when he was struck directly on top of the helmet by a piece of loose material, that was aproximately the size of a grapefruit, that had spalled out of the back.
Injured was removing a rock from the shearer. The rock shifted and cut injured's hand.
THE INJURED WAS WALKING/OBSERVING ON THE NORTH SIDE OF A CRUSHER WHILE IT WAS BEING CLEANED WITH A HIGH PRESSURE CLEANER WHEN A PROJECTED PIECE OF ICE FLEW OFF FROM THE PRESSURIZED SPRAY HITTING HIS LEFT EYE. INJURED WAS WEARING EYE PROTECTION AT THE TIME INCIDENT MINIMIZING THE SEVERITY OF THE EYE INJURY.

EE was on the way out in a battery mantrip along with his crew and a american coal ee. when mantrip became stuck in mud american coal ee got a ram car and pulled the mantrip out of mud he was between mantrip and ram car, the chain was tight he told mantrip driver to pull up about 2 foot so he could release chain. mantrip would not move operator was hit foot pedal.
Emp was climbing onto bolter, slipped and hit face on a piece of screen. He had immediate swelling and numbness to the right side of face. When this did not go away, he sought treatment on 12/09/05. CT scan results reviewed on 12/13/05 with an Ear/Nose/Throat doctor confirmed that he had punctured his sinus, making this a reportable injury.

Employee was moving a ram car through a set of air lock doors. While closing the door employee lost his grip on the door. When the door slammed it struck his ankle causing an ankle sprain.
The employee was dismounting the front end loader at the Sugar Run Portal. As he stepped to the ground he slipped, falling onto his right side. Diagnosed on 12/19/05 with muscle sprain, knee contusion, bursitis of shoulder and arthritis of knees.
THE CREW WAS ATTEMPTING TO CARRY 38 MIL CHAIN TO THE BACK OF THE STAGE LOADER. EE WAS HELPING 3 CO-WORKERS CARRY THE CHAIN WHEN HE FELT PAIN IN HIS LOWER BACK.
A concentrator employee was walking up the stairs, hit his toe on the first step and twisted his foot sideways. He had some swelling and pain but did not go the doctor. Later that evening the pain and swelling became more intense. He went to the clinic, had x-rays taken. Nothing broken but he does have a sprained ankle and will be on crutches and out at least a week.
EE BACKED HAUL TRUCK #2082 UNDER THE 1214 XPB SHOVEL & RECEIVED THE 1ST BUCKET LOAD OF MATERIAL (120 TONS). ON THE 2ND PASS, THE OPERATOR WAS JOLTED BY THE MATERIAL BEING LOADED INTO THE HAUL TRUCK AND SUSTAINED LOWER BACK INJURY. SECOND BUCKET OF MATERIAL WAS ALSO 120 TONS OF OVERBURDEN. OPERATOR WAS WERAING HER SEATBELT.

While employee was cleaning water sprays on miner, a piece of draw rock fell from roof striking his upper back.
Employee climbed 2 or 3 rungs up a 6 foot ladder to grease dryer trunnions. The ladder at top was not making contact with concrete support but rather with a metal rail at the 3 foot point. Shifting his weight above this point caused the rail to act as a fulcrum point and the bottom to slide out from under him. He landed and re-injured his previously injured left foot.
The boat loader leader was climbing a 5 foot steel ladder to get into a flume under the loading shuttle to clear some ice. As he climbed the ladder, two of the steel rungs failed which caused him to fall. He caught himself with his right arm and climbed into the flume. His right shoulder was badly strained and resulted in 3 days off of work.
Haul Truck driver was proceeding to dump site, just after having a break. Truck immediately turned almost 90% from the straight haul path, He hit the berm squarely with both tires and slid down the hill approx. 35 feet. He stated that he saw the truck was turning and heading for the berm, but he could not move his legs or arms to stop the truck.
Employee was grinding when received a foreign body to both eyes.
The injured stated after performing his preshift inspection, he started to mount his rock truck. Upon reaching the third boarding step, the left side of the step broke, causing him to loose his balance and fall to the next rung. It is believed this incident aggravated his groin/hip area that was injured at the end of the previous shift.
Employee was disconnecting belt trailer from hitch - dropped trailer on his left foot causing a contusion. Started missing work on 12/3/05.

The employee was welding on a large project for approximately a week in the same position, and flipping his welding hood up and down. He felt neck pain and sought treatment from Golden Health on 12/09/05. He was diagnosed with 'wry neck' and put off work.
While pulling belt together with red devil, the red devil cable broke and belt clamps hit him on his ankle.

The millwright was walking across the mill 4th floor by the surge bin when he tripped and fell on his left knee causing a bruise. He had stumbled over a 3 inch raised section of concrete in the floor. The injured lost one day of work.

On January 13, 2006, employee began missing time from work due to an injury to the right knee. Injury was originally thought to be a strain, but testing revealed damage to tendon that requires surgical repair. Injury occurred when the employee stumbled on a shovel catwalk.
moving a 55 gallon capacity drum of oil onto oiler's truck, drum raised to bed-level, and then oiler was trying to position the drum in it's proper location to secure it; while positioning drum, rolling it on one edge, oiler complained of lower back pain. placed on tempory total disability 12/30, 31, 1/1, 2; rtw mod duty 1/3
Employee was trying to lift a bearing grease cover to take a temp. check. The cover did not open and he applied more force, resulting in a tendon rupture to his left biceps at the elbow.
Employee uncoupling rail cars, slipped on ice/ mud, hit elbow on car. Cleaned, bandaged back to work. 1-6-05 redness discomfort, none prior to this. 1-7-06, X-ray negative, bony abnormality, aspirated fluid off elbow. Diagnosed traumatic bursitis & tissue cellulitis, restricted duty 7-10 days. 1-12-06, x-ray shows fracture. Lt duty 1-14-06, no use of elbow significant activity.

THIS INJURY DID NOT INVOLE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS OR ATTITUDE. Amendment: Returned to dr 2/20/06-has recurring pain. Back on restricted duty.
Employee twisted left knee by stepping on a stone. Has continued doing his regular job, but injury will become reportable when he has outpatient surgery 2/20/06.
While working in shaft bottom, muck bucket descended down upon employee bruising lower back area. He was taken to hospital and released to return to work. Upon follow up visit on 01/11/06, he was told to remain off of work until next visit on 01/18/06.

The employee was starting up the leach circuit and opened a valve to supply air to the #3 leach tank. Solution came up the standpipe and splashed the employee on his face, hands, upper back and forearms. This resulted in a chemical burn.
