An unintentional roof fall was discovered at 2:15pm at #6xc, #1 to #3 entry, Cedar Grove, Return Shaft area. The roof rock was approximately 6' to 8' high, 18' wide, and 140' long. MSHA and WVMHST were notified in a timely manner.
Fall extended above anchorage point, reported to be 6 to 8 feet high and the immediate roof appeared to be laminated shale with slickensides. Fall occurred at No.17 Break of No.4 Belt between spad 1400 and 1409, between No. 2 Entry and No.3 Entry. Dimensions: 17 to 18 feet wide and 20 to 30 feet long.
A hot idler along with windy conditions caused a grass fire North of the 680 drive house belt line. The fire was not extinguished with 30 minutes.
On the 31st day of August, a roof fall was found in the intake of the #4 Northeast mains. The fall happened at spad #3346 located in the intersection of the #6 entry. The fall measured 20' x 20'. Currently the height is unknown.
While mining a borehole pushed coal away from rib & poured water into section
A roof fall was discovered at Survey Station 24557 in the intake escape way #6 entry going to 002 section. This area was developed in October 2008. The roof fall measured 18 foot wide, 20 foot long and 10 foot high.
A contractor bailing hay using a Vermeer 605 SM Round Bailer. A roller in the bailer fails and heated up. The heat from the roller ignited the grass inside the bailer. Operator noticed the smoke and released the bail. He moved the bailer a safe distances away and unsuccessfully used 2 fire extinguishers in an attempt to extinguish the fire. No injuries in the incident.
A roof fall occurred on 8/30/11 at approximately 11:45pm at cross cut 134 main south entry #3. Fall was in a 4 way intersection that had been mined in Sept of 1995. Area had been supported with 8' full resin bolts. Upon approval from MSHA the fall will be cleaned up and resupported. No injury resulted, but aprox. 50' of belt structure was lost.
The elevator maintenance contractor was working on the buffers for their normal inspection on #6 elevator. Buffers would not collapse property. Dirt around the seals of the buffers needed to be cleaned out, for the buffers to collapse properly.
AT APPROX. 4:45 AM A FALL WAS FOUND IN THE NSM RETURN AT ENTRY # 3 AT X-CUT 61. A STOPPING WAS DAMAGED AS A RESULT OF THE FALL. A J ORDER WAS ISSUED AND LATER MODIFIED TO A K ORDER TO ALLOW US TO PROP ALL 4 SIDES TO THE FALL, BUILD A CRIB ON THE TRAVELWAY AND THEN RESTORE VENTILATION. THE FALL MEASURED APPROX. 25FT. IN LENGTH, 20FT. WIDE AND 9FT. HIGH.
Slips in rock strata caused roof fall in return. Roof deterioration over time making the length of the roof support inadequate. Fall was located in #8 entry at survey station 891 on 1st right main panel.
A fire was started beside Hwy 1 by a fallen public distribution utility line. Due to dry conditions, it spread onto mine property. Local fire department was called to the scene by someone traveling the highway. 0.91 acres were burned. No miners were in the area. MSHA was notified within 10 minutes of recognition.
Fall was found on 2 south mains at 5 and 6 break #2 entry 1 break outby spad #421. Fall was 5' to 6' thick x 60' long x 14' wide. Roof support was 6' torque tension bolts.
On 8/29/2011 a roof fall occurred at approx. 11:18 am. at approx. 100 ft. outby spad #6254 in #2 entry on the A-West Panel. the fall measured 20 ft. by 25 ft. by 7 ft. high.
Midnight shift supply crew was attempting to send out empty co #10 30 ton supply car. Car was placed on A shaft hoist and appropriated stops were in place. Cage began to ascend, car rolled off the skids and wedged between the shaft coping and the cage. Car was removed from the hoist, hoist was inspected for damage and none was found. Hoist was placed back into service.
An unplanned roof fall has occurred at Parker Mains just inby 110 spad in the intake #7 Entry outby seal area. The roof fall was approximately 180' x 18' x 7'. No injuries were reported and no equipment was involved.
A roof fall on #8 Belt was discovered by belt examiner on fire boss run.
The section foreman notified the belt man that the belts had stopped. The beltman traveled up #4 belt to #4 drive. He told section foreman the drive would not start, he traveled up #4 belt and notified the foreman the belt looked like it was bound up. The section foreman traveled to #4 belt and found the rock fall one break inby spad no. 1902. The rock was apprx. 8'H X 12'W X 80'L
A roof fall was discovered in the #9 seam at the 2nd Northeast Parallel xc-5,spad #3+50 in the #6 entry. The approximate size of the fall was length-22',width-18' and height 8'plus.
A roof fall occurred on the 5B L.W. head gate. The roof is heavy over the stage loader for a distance of 30'.
C Panel, #7 entry, spad 11+80 off the Blueberry Hills submain out by section appr. 1000ft. A rock fall was found measuring appr. 19' by 20' by 6' plus high.
An unplanned roof fall occurred on 8/29/11 in the #2 zone #3 entry intersection and through the 3-4 crosscut at spad #699. There were no injuries or equipment damage due to this fall. This area was developed in 12/2006.
Roof fall in McMahon Creek Section at #3 Entry at 33 X-cut. 13' wide, 13' long, 8' high. Water in roof.
A fall of the mine roof was discovered @ 24 Brk in 2 west Mains in the #4 entry. This roof fall does not impair travel or ventilation.
Operating a 637E scraper when a main hydraulic hose broke causing a fire to ignite. The flames from the fire forced the operator to abruptly exit the machine. The machine was left running upon exit causing more hydraulic oil to fuel the fire. Damage estimate is a total loss.

A roof fall was discovered on the inactive 8A section in the #2 entry at 3+40 in an intersection.
A roof fall occurred 50 feet inby spad no. 11201 in the #5 entry in a four way intersection. The roof fall measured 40' x 20' x 10' to 12' in thickness. The area was roof bolted with 3/4"" dia. by 72"" long tension rebar roof bolts and a few 10' to 12' long cable roof bolts. No injuries occurred.
Contractor's scraper had a hydraulic hose failure causing fluid to spray onto a turbo. The hydraulic fluid ignited causing the front end of a scraper to burn.
An unintentional roof fall was discovered between break 33 and 34 in the #2 entry the fall was 30' land X 19' wide X 4'6"" high. This fall will not be cleaned up and is timbered or cribbed on all sides and dangered off. The fall does not effect travel or ventilation.
An unplanned roof fall occurred in the road fork #51 mine in the old mains, #2 entry, 1 brk outby spad #977 that measured approximately 20'long x 5-8'high. No injuries were reported. The area was developed on 3/12/11.
A roof fall occurred above the anchorage point on the C-track at 63 wall. No injuries, no affect to ventilation, escapeways not affected and no damage to equipment.
An unintentional roof fall occurred on the 7 Left Tailgate, #17 Crosscut, #2 Entry, at approximately 2:40pm. The roof rock is estimated to be 70' long, 18' wide, and 6' to 7' high. A portion of an unattended scoop was later found to be under some of the roof rock. MSHA and WVMHST were notified in a timely manner.
#3 production hoist tripped due to a blown fuse. Electricians were dispatched and replaced the fuse. There were still two means of egress to the surface available for personnel underground.
At 11:40 p.m. on 8/24/11 the site lost power shutting down the fuel system. The system was restarted at 3:35 A.M. on 8/25/11. At 8:02 a.m. the coal in the mill self ignited causing an explosion. The mill was shut down and inspected. No damage to equipment or personnel occurred due to the safety dampers properly venting the combustion gases.
After losing power to the mine, a fall was discovered. Area was cribbed and dangered off. No ventilation was interrupted and no equipment involved.
An unintentional roof fall occurred at TG1 break 6 at 4 right. The fall measured 18'w x 8'h x 75'l. It was bolted with 8' fully grouted torque tension bolts.
At approx. 3:00 p.m. a roof fall above the anchorage occurred on the 8 south section in old #5 rm. The fall is in old works and was dangered off on all sides. No clean up on the fall will be done.
At 10:05 p.m. on 8/25/11, the coal in the mill self ignited causing an explosion. The mill was shut down and inspected. No damage to equipment or personnel occurred due to the safety dampers properly venting the combustion gases.
The preshift mine examiner discovered a roof fall above the anchorage zone in the North Mains, Entry #2, Crosscut 18. The fall measured 14' wide x 16' long x 6-7' high. The area was mined in August, 2008. The area was supported with 6' fully grouted resin roof bolts.
An unintentional roof fall occurred on the 7 Left Tailgate, #3 Entry, #11 Crosscut at approximately 4:57pm. The roof rock is estimated to be 50' long, 18' wide, and 6' high. MSHA and WVMHST were notified in a timely manner.
While operating water truck to D3 area, employee noticed fire behind cab. The truck was stopped and fire extinguished after approximately fifty minutes. Off site fire assistance was requested. Damage was noted to the brake system, cab, tires and drive train of equipment.
Grass fire started in non active area of mine, due to tree contacting high line wires during high winds. Approximately two acres affected.
Excavator operator was reclaiming a sediment pond when the excavator bucket struck a gas line puncturing it causing gas to be released in the atmosphere. No injury or equipment damage occurred during this incident other than the puncture to the gas line. This is being reported per instructions from MSHA.
Elevator in #6 could not be operated due to the improper resetting of the over speed governor. A new pin was placed in the over speed governor. The elevator was back in service at 5:15pm 8/24/11
Heavy storms were passing through the area bringing rain, winds, and severe lightning. During this time, it was noticed that smoke was coming from the east. Upon investigation, we found that a fire had started and ignited some dry brush and pine needles. This was located in a remote, inactive part of the mine (S-area).
A roof fall was discovered on this date. The fall was located in the L8-Right Panel in the #1 entry, #28 crosscut. The fall measured 15' by 20' and was 8' in thickness. The area was originally supported with 5' Hy-tech bolts and 12' cable bolts. Ventilation or persons were not affected.
A fire was discovered at 2:55pm in the D2 North mining area caused by a downed 25 kv overhead power line. The fire department was called and the fire was under control at 4:30pm. MSHA was notified of the fire at 3:36pm.
A grass/tree fire was discovered in a heavily wooded area in the D4 area. The fire department was called and a contractor water truck was sent to the location to fight the fire. MSHA was notified of the fire at 3:08pm. The fire was controlled at 4:00pm. CAUSE OF FIRE - Unknown possible cigarette butt or lightning.
A roof fall occurred in the Main West entry #8 at 37 xcut. The fall measured 20'x20'x 3' to 6' thick. This fall did not effect ventilation, passage of men nor did it damage any equipment. The fall will not be cleaned up.
IGNITION: Miner Operator backed Miner out of face on No. 3 Section over to off curtain side to cut face. Miner approximately 20' outby and operator saw a blue flame on mine floor between cat track and pan. The flame was put out within 20 seconds. No injuries were incurred.
A roof fall occurred at spad # 2335 in a 3 way intersection. The area was bolted with 5' & 6' full grouted rebar. It was 5'-6' high X 16' wide X 40' long. It was dangered off and cribbed. It will not be cleaned up.
An unintentional roof fall occurred on the 7 Left Tailgate panel, #1 Entry, #18 Crosscut at approximately 02:43am. The roof rock is estimated to be 20' long, 18' wide, and 6' to 7' high. MSHA and WVMHST were notified in a timely manner.
An unplanned roof fall occurred along the #2 belt from break 26 to break # 29. 20,000' from the active face.
A non-injury roof fall occurred in the #5 entry one break inby the mine portal. The fall was approximately 20ft. wide x 7ft. thick x 20ft. long. The fall was reported immediately to MSHA and OMSL.
We were doing our weekly exam and found a roof fall in our East mains intake in # 7 entry it was a three way intersection at spad # 30+40, the fall was 18' long by 8' high #7 entry is not our primary escapeway.
Three breaks outby spad #2766 in the #6 entry, there was a roof fall in the primary escapeway. The roof fall was located in the portion of the mines that was mined over three years ago. The investigating inspector acknowledged that the mine needed to start doing a more thorough weekly examination. Example: Checking test holes
A Roof Fall was discovered on the MMU. No. 002-0 Section. The roof fall is located in the # 2 entry at survey station # 3256. The roof fall extends outby approximately 130' feet. The roof fall is approximately 20' wide X 130' long x 8' feet high.
Individual was trapped on the elevator when the bode break engaged. He received no injuries and was off the elevator at 11:40. Total time was 56 minutes.
Roof fall occurred at 44 x-cut of Coal Hill #2 on the Belt. Dimension 50' Long - 18' wide - 9' high
#2 Service hoist tripped due to brake pressure fault. Electricians were dispatched to clear the fault. There were still two means of egress to surface available to personnel underground.
At approximately 7:30am the hoist elevator malfunctioned that caused the elevator to travel approximately 12 to 15 feet above the surface landing with 6 miners on the elevator.
A roof fall was discovered on this date. The fall measured 18' wide, 20' long, and was 10-12' in thickness. The fall was located #1 entry of l-& Right at crosscut #13. The area was originally supported with 5' Hi-tech bolts and 12' cable bolts. Ventilation was not affected and there was no persons involved.
Type of support: 6' tension rebar. Cover: approx 200', spad 6219. Dimension: length 40', width 16', height 7'. See attached map. Conditions contributing: high humidity causing detachment on roof support.
An unintentional roof fall was discovered at approximately 12:30 p.m. between the no. 1 and no. 2 entries on the 3 East Mains at break No. 79. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up.

The 8 Haulage elevator was down from 3:28pm to 5:35pm due to the failure of a mechanical brake switch.

The fall was originally supported with 5' Hi-tech bolts and 12' cable bolts. There were no persons involved and ventilation was not affected.
The secondary escape way was blocked for a short period in which interfered with the operation of the brake car.
Fire to a D10 N Bulldozer. No (person) was injured or involved in the fire to the bulldozer.
Man Car lost communications with hoist operator. Replaced microphone on radio in hoist house restoring communications at 12:30pm.
At 1:00am a hydraulic brake pump tripped power due to a loss of hydraulic fluid. As a result the #1 service hoist was inoperable for a period of time until another hydraulic brake pump was brought on line. The hydraulic pump was replaced and brought back into service.
A roof fall occurred behind the wash down bay of the motor pit. It was 3 ft wide by 8 ft long by 8 ft high.
Roof fall 7065 Truck loop BC and 92C. Corrosion of split sets is likely a major factor. Rust stains were observed on rocks near bolts. Area was reported to be intermittently wet since mining and installation of initial ground support in late 2005 / early 2006. K order was issued and a repair plan was put together and approved through for mitigation.

The roof fall is located in the # 4 entry between crosscuts # 3 & 4 approximately 40' feet inby survey station # 7459 of the 2 Left Panel off the 1-East submains. The roof fall is approximately 40' feet long x 18 1/2 wide x 6' high. The roof fall was reported to the MSHA Hotline on 8-17-2011.
An unintentional fall occurred on the 2nd west travelway at the #38 intersection. The fall was due to deterioration of the roof above the bolt anchorage. Fall was 45' long and 18' long and 7' high
Found an unintentional roof fall in #2 Entry at #11 Drive - Fall was cleaned up - Resupported - and supplemental roof support installed.
Elevator Entrapment In 10:38 a.m. -- Out 12:24 a.m. - Employee was stuck in the Concentrator Step 3 freight elevator @ 10:38 a.m. for 106 minutes. MSHA was called at 10:58 a.m. Electronic voltage controller lost power - did not see voltage so the motor timed out, most likely due to dirty contacts. Vendor keeps contacts clean.
A Roof Fall was discovered at approximately 4:00am on the # 5 belt conveyor. The roof fall is located 1 crosscut inby survey station 3781. The roof fall is approximately 60' feet long x 20' feet wide x 6'-7' feet high. The roof fall was reported to the MSHA Hot-line at 4:10am.
A roof fall occurred on the 1st west main intake at break 12. The fall was at a intersection it measured approx 50'long by 35'wide approx 12'high at highest point to bottom.
A roof fall measuring 22' wide, 18' high, 8' thick fell in the belt entry 1000' outby the working section and outby spad 67301. The area was developed 06/12/11 with 6' torque tension bolts as the primary support. The fall occurred due to laminated rock and coal above the 6' primary supports. No injuries resulted from the fall, belt haulage was affected.
Rock Fall occurred on 1 Southeast Mains in #2 entry. Rock Fall measured approximately 45-60 foot long X 20 foot wide X 6-10 foot thick.
Roof Fall discovered at 10:30 p.m. located on #5 belt at #27 Break in #4 entry. The fall was 20 feet wide x 20 feet long x 6 feet high.

Rock fall on 001 section return. Located inby spad #717, on 1 Left off West Mains approximately 3,400 feet from the section. Fall was in the last row of open crosscuts and was between #5 & #6 entries. Approximate dimensions are 20 Ft. wide by 6 foot plus high by 100 ft. long. Fall will not be cleaned up, cribber off.

A non injury roof fall was discovered in the 1st Northwest Mains while preshift exam was conducted. Roof fall was reported to MSHA
A roof fall was discovered in the 5th Northwest Submain xc-18,#3 entry, at spad #14+40 in the #9 seam. The approximate size of the fall was length-40ft,width-20ft and height-6ft plus.
A roof fall occurred 40 feet inby spad 7129 in #1 entry and it measured 6 feet in height, 16 feet wide and 20 feet long.
Weekly examiner located the fall during his exam. It is located at spad 2258
The fire appears to have started and remained contained to the scale house. The structure was a total loss. The structure held the computer, and printer for the scale, tools, safety eq., supplies and all MSHA records. There was no personal injury. Arson is suspected after finding fireworks and tire tracks at the site. Law enforcement is investigating.
An unplanned roof fall has occurred at the intake #6 entry at spad 262. The roof fall was approximately 60ft x 20ft x 6ft. No injuries were reported and no equipment was involved. Lifeline was rerouted and the miners were made aware of the change.
Rock Fall occurred on 2 Left off of 1 Left Mains between Break 14-15 in # 6 entry. Rock Fall measured approximately 60' long X 20' wide X 7' thick.
An unplanned roof fall has occurred at the Old North Mains Return inby spad 6730. The roof fall was approximately 40' x 17' x 4'. No injuries were reported and no equipment was involved. This is a travel way used for weekly checks of seals along the Old North Return. A walkway will be made over the fall using sand jacks.
The slope hoist was down from 6:00 am on 8/14/2011 until 12:40 pm on 8/15/2011 due to the tachnometer on the drive.
During a man trip on the 14' hoist the service power tripped. The battery back-up that keeps power to the program to operate the 14' hoist failed. The program has to be re-installed at this point. The electrical superintendent was called to the mine and reprogrammed the hoist and installed a new battery back-up. The men were hoisted to surface in an egress mode of 30' per minute.
A fall occurred in the #9 crosscut, along #1 belt line. The roof fell above the anchor of the 4ft.resin bolts and comprised 5ft of gray, slickensided shale. It measured 40ft x 25ft and it crushed 4 sections of conveyor structure. The area was posted and cribbed, and intersections from #6 to #9 break were bolted with 10ft cable bolts.
At approximately 8:00 am on 8/13/11 a possible ignition occurred in the A entry of the Rt left section off the 4 left north section. There were no injuries or property damage.
Unplanned roof fall occurred that was approximately 18ft wide X 20ft long X 9ft height in the return airway of #1 section 2 breaks outby spad 1839. No injuries or entrapment involved. Air was not blocked off airtight and will re-route air way.
As the section crew left the section and was in the process of coming to the bottom an individual thought he noticed a fall. He reported that to the Mine Foreman and it was passed on to the safety Director. Him and the production coordinator immediately went to the area in question and did locate a small fall in the intersection.
At approx. 12:40pm an unintentional roof fall was determined in the 2-right section #2 entry from X-cut #103 to X-cut #104. The roof fall we believe is a continuation from the previous roof falls in that area. The roof fall is approx. 120' long, 18' wide, and 8' to 10' high.
A roof fall occurred in the #4 crosscut intersection of the #4 entry. The roof fall was 50' long x 18-20' wide x 5-7' thick. The fall was cleaned up and the area was re-supported. There were no injuries or property damage.
A roof fall was discovered in the old belt entry of the 7A section. The fall was 10'high x 20'wide x 20' long. This area was dangered off and posts were set along perimeter of fall.
A rock fall occurred on Flat Creek 6 Section. It was in the intersection of spad 7+32. It measured approximately 19 ft wide by 20 ft long by 7 ft high.
A driller was drilling on the south side of a loaded face. The driller and charger thought they were working 25 feet apart when the driller accidentally hit one of the loaded holes.
A ground fall occurred on 08/11. This fall of ground was investigated by the company, it did not meet the definition of Accident as defined in 50.2(h)(4) through (12)] and was therefore not reported. Citation 8599535 was issued for failure to report under 50.10(d). We do not believe it is reportable now. However, as ordered by MSHA, we are reporting to terminate the citation
An unintentional roof fall occurred at the New Future Mine 2nd West Headgate Unit Entry #2, 625 West. The fall was approximately 30 feet in length, approximately 17 feet in width and 15 feet thick. The fall did not affect ventilation. The fall will be cleaned up. Fall #28E.
Unintended rupture of 6"" natural gas line. Damage to equipment and electrical components. Incident still under investigation. Cost of damage not yet determined.
Production Hoist Feeder and #2 Shaft Feeder tripped off-line at TR Sub-Station with #2 Shaft Feeder indicating a ground fault. Power for #2 Service Hoist HMI was re-routed. Power was then re-established on the #2 Shaft Feeder at 12:55 PM.

A roof fall occurred in the #9 seam in the 3rd South Panel at crosscut #43 in the intersection of #3 entry. The fall measured approximately 30'x18'x8'. The area was supported with 6' tension rebar roof bolts.
A ROOF FALL was discovered on the 4th south panel. The fall is located across from spad #847 in the neutral entry intersection. The fall measures approximately 60 feet in length, 19-20 feet in width and 8 feet in thickness. The fall does not block ventilation or impede escapeway travel.
On the evening of August 10th the Tracking system stopped communicating to the surface from underground. At Approximately the same time the mine had failure the #2 fan. The repair work to the tracking system started on the evening of August 11th after the #2 fan was restored.
Unplanned roof fall occurred that was approximately 20 ft. wide x 9 ft. High x 20 ft. long, no injuries entrapment, or equipment involved. Blocked walkway #2, #3 and #4 DP. No escape way involved still have flow of air in return. The exact location is #5 panel southeast mains @ spad # 1065.
Unplanned roof fall occurred that are approximately 20 ft. wide x 6 ft. High x (from spad 758 to 764). No injuries or entrapment involved, no equipment involved. Blocked Walkway to #2, #3 and #4 DPP. No escape way involved still have flow of air in return. The exact location is 2nd southeast panel at spad 758.
An unintentional roof fall occurred in the no 5 entry at spad no. 3248. The fall measured 20'long, 20'wide and 4'thick. The fall does not affect the airflow on the travel of mine.

During an electrical inspection the elevator was found to have a bent rod for the overspeed protection. The rod was repaired and put back into service at 5:10pm on 8/11/11.
On 8/9/2011 the 3rd shift fireboss discovered a fall at 7:25 AM. The fall is located between spad #120 and 121 between the #6 and #7 entries. The fall measured approximately 18 ft wide x 40 ft long x 6-7 ft high. This area was bolted with 5 ft fully grouted bolts.
An unplanned fire occurred on the roof bolt machine after welding operations. The initial hot spot check was clear, but upon returning for the second check the fire was discovered and extinguished with 12 minutes.
An employee was riding alone in an elevator when failure occurred. An intercom was used to request assistance. MSHA promptly notified. Service technician was able to float the car and open the door to allow the employee to walk out of the elevator. The individual stated no injuries or illnesses resulted from incident. Time of ""entrapment"" was 6:50 till 8:25.
During an examination of our redundant phone line a roof fall was found in the primary escape way, M4 Mains, XC 64, Entry #6. No miners were injured, ventilation was not affected, and no miners were trapped due to this roof fall.
Flint Ridge Mine #2 has experienced a non-injury un-intentional roof fall approximately 4,500 feet outby the 003 working section on the intake airway in #6 entry at cross-cut #6 on 2 South Mains. The fall dimensions are approximately 30 feet long, 40 feet wide and 4 to 6 feet thick with laminated shale.
Mine foremen went in to check on pump that had been set early in day and found fall in ret.
Reportable roof fall - Main South, #3 entry, #52 cross cut. Dimensions: 60 feet in length, 20 feet in width and approximately 8 feet in height. No injuries. No property damage. Ventilation not affected. Escape ways not affected.

An unintentional roof fall was discovered by the mine examiner on 8/9/2011. Fall in located one break in by spad #1425. Fall measured 30' long by 18' wide by 6' high. Fall will not be rehabilitated.
The hoist tripped out and could not be reset. Seventeen miners were trapped on the service cage for over thirty minutes.

The wana skip hoist elevator went down at 10:30 am with a person on board. The person was safely removed from the elevator at 12:35 pm. electrical problem in the trailing cable, restraining clamp slipped and pulled cable in the control box.
#2 service hoist tripped on a K-1 WatchDog fault. Fault was reset, there were still two means of escape to surface available for personnel underground.
Operator heard a loud pop. When he got out to investigate he found the drill had caught fire. The on-board exting. failed to discharge and a nearby haul unit stopped to assist. The fire was too large and the EE's backed away and called the local fire dept. The fire which lasted slightly less than 30 minutes. A blown hydraulic line was likely the source of the fire.
A grass fire started on the West side of the haul road in Area-C. The fire burned 15 minutes on permitted mine property. Fire was contained and extinguished using mine equipment and local fire departments. Fire was reported to MSHA prior to learning it burned only 15 minutes on mine permit. All personnel were accounted for and uninjured. Unknown how the fire started
The #8 elevator was out of service for a period of time exceeding 30 minutes due to the loss of power from the West Penn Power company and the diesel generator failing. The generator went out of service @ 4:04 AM was switched from auto to manual mode and placed back into service @ 4:45 AM. No individuals were on the elevator at the time.
A roof fall has occurred in the intersection of Flat Creek Main entry #6 entry between spad #36+05 and #37+45. The fall measured 30 ft long by 20 ft wide by 10 ft thick approximately.
Sparks from locomotive started extremely dry grass on fire. The fire started off the mine at approximately 11:20 and spread on to the property at approximately 12:20 p.m.. At that time a Luminant supervisor called the MSHA 800 number to report it had burned 30 minutes on mine property. The fire was located in a non-active area of the mine.
A roof fall measuring 50' long, 18' wide and 10' thick occurred in the neutral entry 500' from the active section. The fall affected intake ventilation. The failure occurred due to coal/rock streaks above the permanent supports, 6 foot torque tension roof bolts. The fall was investigated by MSHA. No injuries occurred as a result of the fall.
An unintentional roof fall was found by the weekly examiner in the 8 Haulage bleeders between the 8H and 8I panels. The fall measured approximately 20' x 20' x 6"" high.

A roof fall occurred on the old main line track at survey spad 304+00. The fall was approx 35ft long 15ft high and 18ft wide.
At 8:50 p.m. on 8/6/2011 the site lost power. The power interruption reset the PLC controlling the ID fan and ball mill tempering damper causing a partial system shutdown. At 9:29 p.m. the fuel system was restarted and the coal in the #1 Ball Mill system self ignited. No personal or property damage resulted from the ignition.
An unintentional roof fall occurred on the mmu 015-0 working section entry #5, 3rd left panel off 8th west submain. The fall measured 25'x18'x6.5'and will be cleaned up. It did not affect ventilation, passage of men, or cause personal injury. The area had been roof bolted with 6 foot fully grouted roof bolts.
At approximately 10:35pm the elevator/hoist malfunction with the DC drive fault causing the elevator to travel approximately 10 feet above the surface landing with 12 miners on the elevator.

Roof did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Area was initially support with 48"" resin bolts. Size of fall 18w x 25L x 4'H. Federal & State Agency's were notified immediately.
On or about 8/05/2011, an unplanned roof fall occurred on the 1st Main North, #4 entry at cross cut #11. The fall measured approximately 18'W x 18'L X 10'H. The area was bolted with 6' bolts and additional support. The fall will be cleaned, and resupported.
A roof fall occurred 120' inby spad #11035 in crosscut between #5 & #6 intake entries. The fall measured 50' x 20' x 6' thick and was bolted with 6' long by 3/4"" dia. tension rebar roof bolts. The area had previously been breakered off with 100 ton Heitzman jacks due to deteriorating roof conditions.
An unintentional roof fall was discovered by the miner examiner. Fall measured 19' wide by 5' high by 40' long. Fall is located in the #8 return approx. 2000 feet from the working section between survey spad #'s 355 & 356. This area will not be rehabilitated.
A fall was discovered at #5 Mains Section (#52 crosscut, #3-#6 Headings). The fall was 100' inby the feeder. The fall did cover the section PC. The area was bolted with 8' Super Twist bolts and 16' cable bolts. No ventilation or people were affected.
A roof fall happened at 9:15 PM in the Main North #1 entry at x-cut 12. The fall is in the return entry and there is another entry to travel and the fall does not impede passage or impair ventilation.

At 3:41pm the #1 hydraulic brake pump tripped power due to a loss of hydraulic fluid. As a result, the #1 service hoist was inoperable for a period of time until the #2 hydraulic brake pump was brought on line. A fitting on the #1 hydraulic pump had failed causing a loss of brake pressure. The hydraulic fitting was repaired and brought back into service.
An AEP 138 kv air switch in the Oak Hill Mine DII mining area experienced an arcing event that caused hot metal to fall to the ground and ignite the grass and trees near the area. MSHA was called at 3:05pm. The local fire departments were called and the fire was contained at 4:45pm. MSHA Supervisor visited the area on August 5, 2011.
A fire occurred on the surface area of the rodeo mine in an unoccupied maintenance building. Damage was limited to this building. Determined to be an electrical short between the walls of the building. No injuries resulted.
On 8/4/11, a roof fall found by mine examiner during weekly exams. Fall was inby spad #7932 off 6 east mains. The fall was in an intersection. Fall was approx 24'w by 40'l by 10'h. Fall was in a intake split of air. Fall does not impede ventilation. No injuries no equipment was involved. Area was timbered and cribbed off. This fall will no be cleaned up
A roof fall was discovered in an outby area in the #1 entry at station number 2809. The fall was approximately 60 feet long x 18 feet wide x 6 feet high.
A fall of roof occurred in the 4th Right panel off the 2nd Southwest Submain, 9-10 xc, at Station 8+90. The fall measured approximately 70 feet in length x 18 feet wide, x 8 feet thick. The area was supported with 5 foot fully grouted roof bolts.
Welding was being performed on the underside of the inlet plenum of the No. 1 Megatex. The plenum is constructed of stainless steel that is internally lined with Tivar 1000 which is a UHMW polyethylene material. The heat created from the welding process ignited the UHMW material inside the plenum. Once ignited, fire propagated through the UHMW material and A side of the Megatex.
A non-injury roof fall occurred at 0530 am on our # 1 belt between survey stations 223 & 253 on our # 1 belt.
An examiner discovered an unintentional roof fall at the #2 entry; Break #9; inby the 8N switch towards Trail Fork. The fall was approximately 20'L x 20'W x 7'H. No ventilation or travelways were affected by the roof fall.

2 employees were in Tilden 2 Pellet Plant elevator when the elevator stopped about 8 feet above the lowest level. The employees were entrapped for approximately 2 1/2 hours. MSHA was promptly notified.
An unplanned roof fall occurred in Panel 1-I at spad #21+70 in #2 entry and extending through the adjacent crosscut to #1 entry. The fall measured 70' long x 16'-18' wide x 8' high. No travel or ventilation was affected.
An unintentional roof fall occurred in the 3 Left Section in the Belt, X-Cut Belt to L1, and the L1 entry at spad 8401. Roof fall was 70' long x 20' wide x approximately 12' high. The area had been supported with 6-foot suprtwist bolts.
A roof fall was discovered on this date. The fall was located on 4-North belt at break #13 in the #3 entry. The fall measured 20' by 20' and was 8' in thickness. The area was previously supported with 5' Hi-tech bolts and supplemental 8' cable bolts. There were no injuries in this incident.
Left rib approximately 25' past the 2750 North on the 5190 East Lateral sloughed out beyond anchorage. The slough was approximately 15' long and 8' deep.
The WANA Service Elevator went down at 8:30 AM with no persons on board. The elevator was back in service at 12:00 PM. The problem was electrical in the control cable.
The fall was discovered at #5 Mains Section (#25 Block, #1 Entry, 2250' outby the feeder). The area was secured with 6' Super Twist Bolts, 12' cable bolts, and cribs. The fall did block the alternate escape way, and the escape way was rerouted. No individuals or equipment was affected.
An unintentional roof fall was discovered by the mine examiner. Fall measured 18' Wide by 6' High by 50' Long. Fall is located between #6 & #7 approx. 400' from the working section across from survey spad #6414. This area will not be rehabilitated. No injuries. No equipment involved.
A ROOF FALL WAS FOUND IN THE MAIN WEST INTAKE # 8 ENTRY X-CUT # 94 AT APPROX. 6:43 AM MSHA WAS NOTIFIED. PROPPED AND CRIBBED ALL APPROACHES TO THE FALL PER INSPECTOR INSTRUCTIONS.
A raised production truck's box canopy contacted an overhead reclaim conveyor, raising the front end of the truck off the ground. The employee was removed from the truck, uninjured.
Roof fall #7 entry - Low cover - mining under hollow on surface - outby spad #1146.
DC power supply for electric brake failed, locking brake on man cage when 150' from bottom coming up with 3 persons aboard. Mine evacuated via production shaft. After PC software troubleshooting, cage operational at 7:20pm to get men off at bottom. Cage fully in service approx. 7:35pm. No injuries or other problems.
A Hitachi shovel caught fire and burnt longer than 30 minutes.
During a weekly examination a weekly examiner found a roof fall in our M4 main. The fall was located in #7 entry at x-cut #71 adjacent to the primary escapeway. The fall did not affect ventilation, passage of miners, or cause injury to miners. The fall was 37' x 17' x 8'.
An unplanned roof fall has occurred at #84 break #3 Track Entry Spad 6906. The roof fall was approximately 20ft x 20ft x 6ft. No injuries were reported and no equipment was involved.
The employee received fatal injuries when he was struck by a rock that fell from the mine roof. He was involved with longwall shield recovery and was building wooden cribs in an area where a shield had been removed. While in the process of constructing the cribs, he was struck by falling roof material.
A miner' s arm was caught in the components of a conveyor head pulley and conveyor scraper blade assembly, resulting in fatal injury.
The victim was leaning over a base lift jack housing. The molded casing catastrophically failed striking the victim in the chest from unknown/undetectable reasons. Cause was catastrophic failure of the housing. Compliance with rules/regs, protective items not a factor, miner proficiency may be a factor, mining equip/systems may be a factor.
The employee was using a table saw to cut pieces of PVC blocks. With the saw running, he reached under it, and his right middle finger was struck by the rotating blade. He sustained a laceration and damaged distal phalanx bone. Prior to the task the supervisor had a meeting with the three employees that were assigned to do the task about the safety issues with the job.
Mechanic was dismounting haul truck when he twisted his right knee- employee stated he must have stepped on a rock or clod of dirt. Leg was put in splint and he was issued restrictive duty.
Employee was trying to install a sharp pin on a stamler feeder the bar he was using kicked back and pinned his right ring finger against the frame of the feeder mashing his right hand ring finger, cutting his finger off from the last knuckle out.
While traveling the intake on a mine buggy to work on the tracking system a piece of draw rock fell striking him.
The employee was removing a piece of waste granite from a saw cart using a steel lifting cable. The employee put pressure on the waste piece with a pry bar to allow the lifting cable to go around the waste piece. As he applied pressure a piece of the waste slab broke off and fell striking the employee on the left shoulder causing an abrasion and laceration.
An EE gave IE a pocket knife at their lunch. EE lobbed knife from his loader to IE, who was in his truck. IE reported to owner that he cut his hand when he caught the knife, which must have been opened when tossed back to him. IE went to ER and there was possible tendon damage and ER temporary closed wound. IE had surgery on 9/5/11 to reattach the tendon and repair nerve damage.
Employee reported continuing knee pain after his foot slipped on a clump of dirt while walking on blast pattern. He was referred to a specialist who diagnosed him with an extensively torn and degenerated meniscus and mild to moderate chondromalacia. Surgery was performed on 10/11/11.
Was grinding on rock truck to prepare for welding when grinder hit nose.
Employee was roof bolting and twisted right knee
The EE was walking through mud, stepped on a section of slick line causing him to fall. He caught his weight on his right shoulder. On 9/15/11, MRI diagnosed possible labral tear. He continued to work his regular job as a shuttle care operator with occasional pain. On 12/15/11, a MR Arteriogram determined a labrum tear. The EE's fist day off was 1/16/12 when surgery was performed.

Employee does not exactly know how or when the injury occurred. He believes that it occurred while walking in the mud pulling vent tubes. Employee worked the following two shifts of the week 09/01 & 09/02 for a total of 27 hours.
Employee was climbing off grader and trying to stop door from closing when he twisted his left knee.
Employee was viewing the #5 locomotives wheel location when the motor unexpectedly dropped off the rail striking the employee's right leg, resulting in compound fractures.
Employee was disassembling brace for a conveyor belt legs when the brace swung down and hit his foot after he loosened one end.
Operator was standing on track of dozer when his foot slipped and twisted his right foot - hair line fracture.
EE was standing on the ground to the left of the motor grader using a tripod level. Grader EE backed up. The operator of the motor grader was backing up and looking to the right and did not realize that EE had backed up. He turned the wheels sharp to the right and the front tire struck EE.
The employee was seriously injured when he was caught between the rib in the #9 heading and the #2 roof bolter machine being operated in the No. 1 North East Main Section. The bolter was hung up and was being pulled by a scoop. The roof bolter slid to the left pinning the employee. The employee sustained a fractured pelvis and required hospitalization as a result.
Employee parked the dozer on a small incline, stepped out onto platform then onto track. Dozer rolled backwards pinning employee's left foot in between dozer track pad and the platform. Employee fell onto the track then onto the ground. Employee did not set the parking brake. Employee injured left foot.
EE had a set of come a longs hooked to a roofbolt plate. The plate broke causing the come a longs to fall and strike him in the side of his head above his right ear. This caused an egg sized knot and cut that required stitches. No fractures or other injury.
Employee was putting oil in loader when he lost his grip on the hold bar of his loader. He was able to grab on to the loader without falling, but in the process he dislocated his right forefinger.
Individual was repairing a shuttle car splice and while cutting the cable felt pain in the right shoulder while handling cable. Worked until 1/12/12 when individual went off for shoulder surgery.
He was running a tractor pulling a scraper at the Shamrock Mine. He was wearing a seatbelt and not driving at any excessive speed. He crested the ramp and hit a rut. The bounce that it cause made him sprain his back.
Employee was picking up oil can and felt something in his shoulder move.
Employee was operating a 973 track loader to move some clay. He developed a pain in his neck. He went to the doctor and was diagnosed with 2 herniated disks and returned to work with restrictions.
Employee was connecting black water line together when the water line slipped out of the coupling striking the employee in the mouth, breaking a tooth.
The employee was removing angle iron framing and wood from a MCC building. The employee used methods of drilling and cutting during the task, and all work was above the employee's head. The employee was wearing a pair of safety glasses at the time of the incident. A piece of debris flew in-between the employee's safety glasses and forehead and lodged in his (R) eye.
The employee stated that he was down on one knee tightening a fitting to a new section of water line he had just installed to the miner. As he put pressure on two pipe wrenches he stated he heard a pop in his lower back and immediately felt pain and tingling sensation travel down his left leg. The ee began missing work on 8/30/11 and was diagnosed with a lower back strain.
The employee alleges he was thrown/knocked from the tracks of the dozer, while he was relieving himself, when a 777 truck bed came into contact with the dozer blade at the dump site. The company has reason to believe that this was a staged accident and will contact the workers comp claim.
The EE was attempting to remove a rock in the sampling unit. The electrical circuit to the belt had been de-energized, but not to the coal sampling machine. The EE got between the belt and the sampling arm. The sampling unit activated by a timer came on pinning the EE with the metal arm. EE was flown by helicopter to Holston Valley Hospital. MSHA hot line notified on 8/29/11.
Employee was inside the cowl of the Poclain, bleeding the hydraulic pump. He leaned over the PTO and his clothing was caught, immediately pulling employee in.
Continous miner was turned off. Employee went to move miner cable, rock fell off miner hit left foot
Driver was walking from truck to loading facility when he stepped on rock twisting left ankle.
Employee was stepping up on to a walk thru roof bolter when a piece of rock measuring approximately 32""l x 17""w x 2""t fell on his left shoulder.
Miner was sinking shaft when his partner knocked a timber down from the landing just above him, striking him in the head causing a mild concussion.
Degenerative cervical disc, vibration over period of time coupled with previous history of neck problems. Employee should be able to return to work without restrictions when rest and therapy sessions are completed.
Employee was assisting the loader operator in cleaning up supply yard. As the loader operator was scooping up mud and debris the bucket struck the end of a timber causing it to swing around and strike the injured employee in the legs causing him to fall. As he fell he extended his right arm out to break his fall causing his shoulder to dislocate.
Employee's left index finger was pinched in a hand grab railing while attempting to enter the cab of a haulage truck, resulting in a fracture injury.
Employee was changing fuel filters in 980G loader when the hood fell on employees arm hurting right wrist.
Tripped and fell while walking along side a scoop landing against the rib on his left shoulder. Worked until 2/6/12 and had rotator cuff surgery.
Employee was fuelling up a pump used to de-water pond. Whilst doing this he slipped due to wet ground and twisted his right knee.
Employee was helping build cribs and some small wood was needed to cap and wedge the last crib. Employee was using a block hammer as a chisel and a pick hammer to drive the block hammer in between nailed wood of a pallet and as the employee struck the block hammer with the pick hammer, a piece of metal flew off of the pick hammer and struck and embedded in his left arm.
Employee was pulling and pushing on 3"" solid water line when he felt a pain in his right calf muscle.
A heavy equipment mechanic had just completed maintenance on a Caterpillar 777D and moved the truck outside onto a concrete staging pad outside the shop. The mechanic exited the cab and decsended the driverside ladder to the bumper level when he lost balance, traction and grip. He landed on the concrete floor, his left heel taking the force of the fall.
The injured employee was using a slate bar while working with a crew to repair a breaker car. The bar slipped and struck him in his right forehead. This resulted in a laceration that required medical treatment.
Employee was lifting a belt roller with a slate bar while another employee was trying to adjust the height of the roller. The slate bar broke striking the employee in the groin area.
Employee was driving coal truck, when he went into the ditch causing the truck to turn over and the coffee thermos came up and hit the employee in the upper lip.
The mechanic was in the middle of fixing the foot switch on a golf cart when he dropped his supplies on the cart engaging the tram pedal. EE was pinned between the golf cart and mantrip, deep bruising his right thigh. He was taken to the Doctor and was put off work until Sept. 2, 2011.
Injured slipped cleaning rock from chute. Said he twisted right knee but it was not hurting enough for attention. On 11/9/11 injured said knee was bothering him and went to Dr. Dr. told him to give it time and it may heal itself. Continued to hurt, returned to Dr on 2/29/12, Dr scheduled medical procedure, on 2/29/12, management became aware of the reportable injury.
He was servicing the miner. The miner bits got caught on the line curtain, so he shut the miner off and tried to get the curtain loose. Since he could not free it, he started the miner to drop the head. He placed his left hand on a bit lug. Instead of lowering the head, he mistakenly raised the head, smashing his left hand between the roof and the miner head.
At the end of the crew meeting, miner stood up from a seated position on a bench in the safety huddle room when he began to have pain in his low back.
EE reports he was shoveling on the tailgate of the long wall when they heard some rock falling and took off running and tripped over one of the shoes on the longwall system falling and straining his neck and back.
Individual was dropping ventilation tubes when a ventilation tube bounced off the rib and struck the left knee bending the knee backwards. Individual continued to work until 9-20-11 when he had surgery to the left knee.
Operator went to the back of the bolt machine and picked up a box of resin. He threw the box onto the machine and felt a pull in his left shoulder. He continued bolting and finished that cut. He left work and sought medical treatment at Raleigh General. Drs there advised him to take 1 day off. He RTW on 8/29/11 after missing one scheduled shift. He did not report this until today.
A rock measuring 18"" x 36"" x 1"" fell from approx. 7' striking the employee on the head and left shoulder. Employee became a lost time injury on 08/30/2011.

EMPLOYEE WAS UNLOADING SALT FROM A RAILCAR AT THE UNLOAD STATION. LARGE CHUNKS OF SALT BEGAN TO BUILD UP IN THE UNLOAD CHUTE. EMPLOYEE USED A 1/2"" PIPE IN AN ATTEMPT TO DISLODGE THE SALT AND THE PIPE CAME INTO CONTACT WITH THE AUGER. WHEN THIS OCCURRED THE PIPE WAS FORCED TO THE NORTH AND THE EMPLOYEES RIGHT INDEX FINGER IMPACTED THE BOTTOM OF THE RAILCAR TRAP.
Employee was lifting a piece of screen sire and felt a twinge in his neck and down his left shoulder.
Employee was setting a timber. He got down on his knees to set the timber. While on his knees, he twisted around and said he felt his knee pop.
Employee was unselective carrying metal roof straps (roof-bolting supplies) haphazardly in the supply area outby working section when he tripped over metal roof straps that were lying on mine floor. Employee fell down and injured his lower back as a result of tripping and falling.
EE was using torches to remove the cable stand off the right continuous miner. While cutting, hot slag went into his left ear. He continued working until Aug.28,2011. He sought medical attention on the third day after becoming light headed. Doctors said he had burned his eardrum and the ear was infected. He is off work receiving treatment.
Employee was working out of the bucket of a 3.5 loader when he slipped and hit his right knee on bucket. Employee did not start missing time from this injury until 10/21/2011.
Ee is a repairman. He was preparing a piece of equipment to work on it. He was replacing the cable. When he threw it over the equipment to the side he pulled his back. The doctor is doing tests. Too soon to say how long he will be off.

EE was standing on bulk bag scale when he slipped and twisted left ankle.
Individual was greasing the lift arms on a 988F Caterpillar loader. He slipped and fell to the ground about 18 to 24 inches. Reported that he twisted his knee. He did refuse treatment and or to go to the doctor as noted on company sign off accident report.
Employee was helping another employee hoist a swing gear lid from the back of a pickup. The hoist chain safety latch broke and the lid fell, knocking the employee over and landing on his feet. The accident resulted in multiple fractures to the employees left foot and a sprain to his right foot.
While pulling rock off of the Continuous Miner with the scoop, the rock broke and landed in the scoop bucket bouncing the scoop operator (victim) against the roof.
Employee and co-worker were hanging miner cable. As employee lifted the cable he stepped backwards and mistakenly stepped on co-workers foot causing the employees right knee to bend awkwardly and caused him to fall down on mine floor. Employee had an MRI on right knee and is waiting on results.
Boxley Employee was walking to a switchhouse to start a crusher when he rolled his ankle on smooth level ground. He reported the injury to his supervisor and due to swelling he was taken to see a local physician. It was determined by the physician to be a sprain.
EE cleared a rock jam in the crasher. He then restarted the conveyor to the crusher and was standing by the conveyor when a rock fell off the conveyor, strick the door above ee which then hit him on the head and shoulder.
Employee drove front end loader into a dip in roadway causing the loader to bounce which caused the employee to strike his abdomen on the steering wheel and along with tension from the seat belt resulted in an aggravation of an off duty injury.
Employee went to the well to get a water sample. Tried to start the pump by turning on the breaker. He opened the door to the breaker and saw the breaker was tripped, so he tried to jump the breaker using a jumper
Following a 5.8 earthquake at 1:51PM the dry plant had a lot of dust fall from the rafters & visibility was low. EE was hurrying down a flight of stairs in the excitement & low visibility from the dust, he missed a step & when his foot hit the floor he felt a slight pain in his back. At home his back worsened & he was taken to the doctor & diagnosed with a back sprain.
A small piece of rock fell between roof bolts, hitting the victim on the upper left shoulder and knocking him to the ground. The victim received bruising to his left shoulder, upper back, and laceration to his mouth. The laceration caused from being knocked to the ground, not the rock itself. (His teeth cut his mouth)
Employee was swinging a sledge hammer breaking up a rib roll when the sledge hammer got caught up in some tie wire hanging from the roof and struck himself in the head.
Employee was walking down slope, slipped and fell causing compound fracture of right ankle
The driver climbed up the side of his tri axle dump truck and slip and fell off the ladder resulting in broken ribs and a separated shoulder.
While opening the magazine door the top lock stuck EE pushed on the top of the door and bumped the bottom of the door with his left foot at the same time He then felt a pain(burning) in his lower abdomen/groin area and it began to swell The hospital released him back to work and had a checkup appt. on 9/1/11 at which it was determined to be a Hernia requiring surgery to repair
Injury occurred while the employee was stepping up a small, weedy embankment after assisting his supervisor in repairing a flexible discharge hose on a 3"" pump. Employee stated that he felt pain in his left calf muscle area and had difficulty putting his body weight on that leg.
Employee working in possible 'bite' area if steel should break away from it's mount while forcing with come-along. Damage was to one of the employee's knee (right)
Employee was changing out bad rollers along the #3 min line belt. EE was dragging a roller along the belt line while walking backwards and he tripped over another roller which had just been removed from the belt line, resulting in the emplyoee spraining his right ankle.
Greaser walking to his truck felt a sharp pain in his left calf. Office secretary drove him to the hospital with a pulled muscle in left calf.
Roof bolter operator was bolting in #5 right. He was operating the offside drill and as he swung it out to drill a hole, a rock (11""x10""x3"") fell off his canopy, striking his left shoulder. He complained of numbness but received no abrasions or bruising. He refused ambulance care and drove himself to Raleigh General where tests were negative. The doc gave him 2 days off work.
On the 002 section, in the No.3 entry, EE was finishing loading his shuttle car. Once loaded, EE stuck his legs out of the car to turn around when a rock dislodged between two bolts from the mine roof. The rock struck EE's right thigh causing a fracture to his right femur.
EE was roof bolting in the #4 entry on the MMU. As the EE was drilling the hole, the coupling that connects the drill steel together broke causing the 2 sect.of steel to come apart. The upper piece of drill steel fell out of the hole which bounced off the drill canopy striking the EE in the lower left ribs, caused a small burn on upper side of the left bicep,abrasion on left ribs.
While going to answer page phone slipped on piece of rock & fell & twisted his back.
The employee dropped a striker onto a belt from the jaw. He climbed onto the belt to get the striker. While another employee turned the belt on to run the striker out. This resulted in what we assume to be a broken ankle. The jaw and feeder were locked out but not the belt below.
Employee strained his back due to being bent over while welding.
Employee was working on drill when he started to get up, his foot got caught and he twisted his left knee.
While getting off the dozer he slipped on step and twisted his lower back.
While opening the cone crusher a trapped rock flew out and hit employee on the arm and on his face. There was a bucket tooth lodged in the cone and it had stopped the rotation of the cone, so the outer liner was raised to free the tooth.
Employee was attempting to cross a stopped belt when he fell injuring his right shoulder.
Employee was using a lifting jack while working on a shuttle car and the jack handle broke, striking his arm/wrist and causing him to fall on his arm/wrist.
While take a cut piece of marble off of the block marble had a crack an fell a mashed his fingers
When ee and coworker were removing the tail pulley guard he stepped over uneven ground and lost his footing. When he stumbled he caught himself not completely falling and hurt his back. Employee has worked full duty with some back discomfort until 3/09/2012 when he had surgery to alleviate the pain. - This surgery makes the incident reportable lost time 03/12/2012.
Lower back muscle strain while installing trans pump.
Employee went to foreman on 8/23 and stated he had hurt his back on Sat. 8/20 lifting a 5g pail of oil while doing a service. The employee left on the morning of 8/23 after telling the foreman of the incident. The accident was not reported until three days after the alleged incident. Employee states there were no witnesses and not sure how it happened
EE Was reaching for vacuum hose when his left arm was caught in hose resulting in swelling and abrasions.
Employee was walking down a hill and slipped in mud, his left ankle twisted. He said he felt it pop and sharp pain. Employee was transported to HRMC ER at Prestonsburg Ky. X-rays were negative and was told to follow up with family Dr. His Family Dr. Examined him and took him off work till 8/30/11.
Individual was walking down 15W track entry when tripping in an off set in the mine floor. Falling on right wrist. Individual continued to work until 8/24/11 when he went off work due to incident.
The injured has exited a scoop when he twisted around to walk and fell resulting in a broken leg
Employee was moving a miner from the face into position for the next cut. A rock fell from between the bolts and struck the employee on the back. He fell to the ground and rock hit his left heel as well. Subsequent testing revealed two cracked spiny member od L1 and L4 vertebra and a fractured heel. Employee was taken off work on 8/22/2011.
Employee was descending a flight of stairs, when the right foot slipped out, and his right heel caught on the next step below. The result of this slip was an injury to the right knee.
Welder gouging steel on blade, spark caught his coveralls on fire, burned through coveralls and jeans on left leg, middle of leg between knee and foot.
The trucks bed started sliding around to the left and he tried to correct it. His R/front tire struck the berm on his right side causing the bed of the truck to turn on around and his L/front tire struck the berm. This caused truck to turn striking the left berm below him with his left front tire causing the cab of the truck to overturn.
The injured employee was operating a shuttle car and was traveling from the miner to the feeder. The injured employee stated that he hit a bump and the brakes locked up on the shuttle car ejecting him from the car.
He was mounting the jaw die of the jaw crusher, they were set with a wooden piece, opened the jaw crusher to align the screws, apparently one of the wooden pieces fell causing the jaw die to fall forward and wedge his right foot.
Employee was walking down the steps from the highwall miner launch and when he stepped off the last step onto the ground he stumbled and twisted his right ankle.
Employee previously complained of pain in finger of right hand while operating a weed/grass trimmer. He was promoted to a job with no vibration and stated that he again had pain in the finger so he sought medical treatment. He was given three days off and returned to full duty.
The employee reported jarred by shovel when bucket hit truck
Employee was working to rewire VFD control box for electric booster station pump. Employee reached inside the VFD circuit box to check the wiring and must have hit a bare wire causing an ignition or spark to occur that caused him to have burns to his face/head/neck/arms.
Employee reported injured back while shoveling feeder.
While in the process of removing the locking pin from the Case 580M backhoe, an employee was pinned between the outrigger and boom causing injury to the employee.
Injured was riding out in mantrip with his left foot outside the trip. As mantrip went around a supply sled his foot caught between the mantrip and the sled bending it back.
Miner was removing lower dampener housing bolts air ratchet got hung up and when he pulled on hose ratchet to free it ratchet came up under his safety glasses and hit his right eye
The ee alleges an injury to his back while dismounting a personnel carrier. On August 18th the ee denied any event that caused injury. The ee left the mine and mine property on his own without medical assistance.
EE states that he was installing a grate in floor at the truck garage, he was removing a piece o metal grating off the truck when the metal grating slipped and struck 2 fingers on his hand.
Employee was setting breaker timbers and the timber fell and struck him in the back.
Employee was jacking dollies back on track on the mainline track. Over a period of time throughout the shift he felt tightness in his back. He went to doctor and MRI appointments and began to miss work on 8/31/11.
Ee is a dozer operator. He back over a rock that was not in his eyesight causing a jolt of the machine. This jolt caused him to be jarred severely causing a hurt back. He missed two days and is not back to work with very little problem.
Employee stated: Was shoveling ""heavy"" mud - speculates he twisted wrong way and went down w/""excruciating"" pain in back - concentrated in middle to lower back.
Injured releasing grease take-up to replace CBM Foot Shaft when the grease gun fitting he was using broke allowing pressurized grease to strike him in the face. Employee has been working his regular job, but had surgery on 9/15/2011 to remove imbedded grease from under the skin on his face. He has not missed any work as a result of the surgery and continues to do his regular job.
Victim was cutting a clip off the belt when he slipped and the knife cut his hand.
EMPLOYEE WAS LIFTING A PIECE OF STONE AND STRAINED HIS BACK
The mechanic was holding the sides of the planetary cover of wheel assembly and putting it in place onto the housing when his right mid-finger was pinched.
Injured employee was standing in between two (2) mobile rides, when one (1) of the rides was struck by a scoop, knocking the rides into each other, pinning the employee causing a fracture of the right lower leg.
Walking down long wall face and lower back gave out. PPE was not a factor, Compliance of rules/regs not a factor, mining equipment/system not a factor, Cause is not being aware of mine floor conditions when walking on long wall face.
Employee pulled muscle in shoulder while pushing a wacker pump onto the tailgate of a pick-up. Employee reported the incident and was taken to the company physician. Employee was released to return to work. After experience more pain, the employee was taken back in on 09/8/2011 and was discovered to have a torn tendon in rotator cuff which will require surgery to repair.
Employee was replacing a fuse in the jaw disconnect box. After throwing the knife in the box, the employee used a fuse puller to remove the bad fuse. When replacing the fuse, the employee got the fuse holder into live electricity and got flashed.
Employee was helping get a rock off the miner -had pain in lower back.
Employee does not know exactly how he received his injury. Dust or Dirt got into his eye and was treated by doctor.
Employee was using a pry bar to dislodge some material in a hopper when the bar slipped, causing a quick jerk to his back. Diagnosis is pending.
Employee alleges he was taking apart a piece of equipment and a metal side fell and hit the basket on the man lift. The man lift jerked and caused him to hit his face on the handrail and caused a fracture to his face.
EE was trying to remove a large rock from the tail of the Joy 14Cm15 by lifting the tail section to the roof and dragging rock off. A piece of the rock broke off and flew back 8'-10', striking the miner operator on the left lower leg and ankle.
EE worked on 8/17/11. Did not report any injury. EE called on 8/18/11. He stated he had something in his eye and was going to get checked out. Upon discussing incident with ee he stated he did not know when Dr where he got something in his eye. EE wears prescription glasses at all times. The company protested claim to workmans comp but was denied on 9/30/11
Employee was climbing down the steps of the wheel loader he had been operating when he began to feel pain in his lower back area.
Employee and coworker was lifting handling an I Beam when he strained/sprained his pelvis. Employee went to a doctor in Mt. Carmel, Illinois and was diagnosed with a hernia. Employee had surgery on September 08, 2011, in which at that time became a reportable accident and will be off on Workman' s Comp for an uncertain period of time.
While employee was tightening bolts on the Plant Feed Belt he felt a sharp pain on his left side. Employee was elevated and tied off with his safety harness. He was using his right hand to hold ratchet and left hand holding onto belt structure. Employee was treated at Raleigh General Hospital and was diagnosed with bruised ribs.
While holding cables to hand up, stepped off small ledge, left knee bent backwards.
Employee was installing a shear pin in the sheeps head on the relay bar at shield #152. The relay bar pushed out shoving the shear pin into the pan line. Three fingers on employees right hand were caught between the shear pin and pan line.
EE hooking bag strap to arms on Bagger 150. Walked across platform rolls to hook other bag strap-slipped-grabbed bag for support injured left wrist. Did not seek medical attention on 8/16/11. On 11/21/11 sought medical attention was given restrictions - NYCO able to accommodate. On 1/19/12 EE placed out of work - NYCO no longer able to accommodate restrictions.
Employee was cleaning ditch out shoveling and strained his back.
The employee stated he strained his left knee stepping in mud. The event has resulted in a lost time injury.
Employee was a passenger in a Jeep (Jitney). The Came off trolley pole came off the trolley wire, struck on a roof strap and was pulled out of the base. The pole struck the employee on his left elbow.
EE was working on a dozer and while straightening out a hose on his wire feeder he tripped over the track that he did not see that was behind him. He fell hard to the ground and hit his right knee, he was taken to the Brackenridge hospital for treatment.
While putting anti freeze in the loader, the employee stepped on a loose metal plate and twisted his ankle.
While lifting a roll of cord onto loader, EE pulled lower Back employee claims. He also advised this is pre-existing condition and he works another job. (approx 20 lbs)
Individual attempting to free a stuck drill steel, had right hand on or in the vicinity of steel. Left hand was on the control levers, apparently on the wrong control putting up pressure on the steel. The steel bent, catching the right hand between the steel and bolter mast. Individual suffered a broken index finger and sutures to the middle finger.
Employee was welding and fabricating on a pump box. Some time during the night (early morning)a piece of metal dust/sliver got into his right eye.
While working on belt line a guard over the wheel caught employee dragging over leg and pinching him between guard and the foot of take up.
Employee was adjusting motor base while using pry bar slipped causing employee to fall on left ankle causing injury.
Employee alleged that he stepped in hole fell down and twisted his knee while walking conveyor belt line, no witness to incident. The employee alleged the area was off #2 section at 6A head at spad #1385.
EE alleged experiencing stomach muscle strain after lifting on roof bolter T-Bar. Did not start missing work until 8/18/11.
While installing belt rollers, the employee was pulling hard on a wrench to tighten the bolts and strained his right shoulder. At a later date, it was determined he had torn his rotator cuff.
Operator got out of his machine and picked up a broken shank and tip. He hoisted the broken piece onto the catwalk of the machine. Employee stated he felt pain in his groin at this time.
Injured employee was pushing a dead coal hauler out of the way with his ramcar and the hitch on his went into the bucket of the one he was pushing. This car went over a small hill in travelway and raised his off the ground. When it came loose and his coal hauler hit the ground it threw him up into the canopy of his ramcar, causing multiple injuries.
Traveling out of the pit, antifreeze was seen coming from the radiator. The operator placed his hand on the radiator cap to tighten it, in doing so it came off getting hot anti-freeze on his arm and leg.
Employee was changing a 3"" hose on a cement pump. His right thumb was pinched and fractured by the brass hose coupling.
Employee was lifting rock off of the continuous miner to gain access to a broken conveyor chain to be repaired when injured his back.
While measuring the height of a head roller on the 3b installation area, the employee was struck by the portable head roller boom.
Employee twisted his back while pulling on a 1"" water hose.
Checking on trap on field conveyor slipped got hand between conveyor belt & roller on conveyor.
Employee was standing on a ladder and retrieving a cinder block from another employee. As he was lifting the block he twisted towards the wall to set the block in place. Strained abdomen in process.
Crew was working on screen unit. EE was working on west side at the discharge end of the screen unit. He was reaching to tighten a bolt on the mahcine, when he was finished he stepped back and fell off the end of the cat walk and fell to the ground 6 feet. He landed hitting his head on rocks on the ground.
Employee was assisting in replacement of cross flow panels when employee lifted one side of panel felt pain in left forearm. **EMPLOYEE HAS WORKED FROM TIME OF INJURY UNTIL SURGERY 10/17/11 ON ULNAR NERVE IN ARM***
Employee was lifting a piece of equipment and was struck in the face by the rigging.
Employee's dental appliance came loose during loading causing an injury to an anchor tooth.
Employee was checking water level on equipment. He unsecured the cap with his foot & it had no pressure on the cap so he removed the cap from the radiator with his right hand. About the time he raised up, the radiator began to blow hot water out & hit his right arm, face, back & right leg.
Employee was operating #17 man bus when trolley pole came off of trolley wire and struck him in the face. Began losing time 8-16-11.
While taking the slack out of the Joy Continuous Miner conveyor chain, he was standing on the conveyor deck between the flights at the back of the boom. He was hitting the conveyor link with a 3 lb. sledge hammer when the link came apart. As the chain slid off the tail roller it caught his left foot. He fell 4' to the bottom landing on his left side.
He claimed that he got finished cutting the place he was in and got up to tram the continuous miner, then his knee popped. During the time his knee popped he stumbled then twisted his lower back. This we believe should be a medical issue from a previous old injury and should not be a MSHA reportable injury.
While EE was operating water truck, employee said brakes would not hold & collided with 100 ton rock truck head on. Damage to water truck with no damage to rock truck. Rock truck driver sustained NO injury.
A contractor was making modifications to the exhaust ventilation pipe on the 59-3 GOB ventilation borehole with the 9 stage methane exhauster pump operating. The modifications caused methane to be passed over a hot surface causing methane to ignite which resulted in burns to an employee servicing the methane exhauster.
Employee was hanging curtain. A piece of coal came loose from the rib and hit him in the head and left shoulder.
Employee was getting off of drill using the ladder when he stepped off onto an unstable rock causing him to twist his knee
Employee was operating chain saw cutting trees. While cutting a tree the chain saw kicked back and employee lost control allowing the saw to come in contact with left arm causing laceration.
EE was pulling belt around corner of a clock and went to rehook chain on belt when a piece of rock fell and struck him in the leg.
While Employee was raising the drill pod of the roof bolter, to push bolt up, a piece of draw rock fell striking employee on his hard hat. Employee was treated at Raleigh General Hospital and x-rays were taken.
After fueling equipment, employee went back to his pickup waiting for everyone to show up for work. He was eating McDonalds, started to choke on food, got out of truck to get a drink of milk and fainted. He fell on ground, face first. Co-worker heard him yell for him and found him laying on the ground.
EE was being transported to the mine bottom on company #73 bus due to an illness. During transportation the bus came off track at 1 South #1 spur switch causing the EE to strike his head on a metal cover located behind the operator and strain his neck.
Employee stated that he slipped while stepping from roof bolting machine operator platform to the machine platform and fell and struck his back on machine.
Employee was putting scoop on charge when he twisted his right knee.
A piece of draw rock fell from top and hit Ee on foot and ankle. Went to doctor to get checked out. Nothing broken, just strains. Ee went back to work after one day off
At 9am driller observed EE's knees buckling and going to the ground. EE insisted being fine just tired from lack of sleep. He finally agreed to be driven to hospital. He was admitted for heat exhaustion and severe dehydration. Later diagnosed with knee contusion and hairline ankle fracture from fall. He was released 8/16. His return to work is pending medical release.
EE was picking up a empty wood pallet and felt pain in his lower back.
Ee was installing a V-belt on a motor when his finger got caught. This mashed and cut off the end of his finger. Took a few stitches and Ee missed two days work (Friday and sat).
EE was bolting top and moved a heavy rock, Supervisor stated that it looked like a hernia in the left groin area. EE refused medical attention and refused ambulance, he also continued to work regular duties thru 8.31.2011. EE notified us that he will have surgery on 9.1.11
On stepladder leaning against rear of excavator looking for oil leak when machine shook the ladder and caused him to fall and striking the ground.
While lifting a rock with comealongs, a rock fell and struck employee on the hand
Employee was getting out of yard locomotive to throw switch and when he stepped down he twisted his right ankle. Sought medical treatment on 8/14/11, was examined & given full release same date. Employee stopped working 03/05/12 stating that he continues to have pain in ankle and stated that he did not have any other incidents that contributed to condition.
Employee's left foot was pinned between ride and rib when tram stuck in.
Miner stepped off a 22"" high platform injuring his right knee. He has had pain and swelling but did not seek medical attention until 09/13/2011.
Employee states he was helping unload belt structure from a trailer. He positioned himself where his right foot was placed in front of the trailer wheel. As the trailer moved forward, it rolled over employee's right foot. Employee received a crushing/laceration injury requiring three stitches.
EE WAS WORKING DAYSHIFT AT MINE OPERATING A LOADER(l220 VOLVO). EE EXITED LOADER, WALKED TO REAR, STOPPED,MADE ONE STEP BACKWARDS AND HIS RIGHT HIP FRACTURED AND HE FELL TO GROUND. THE WALKING, WORKING SURFACE WAS EVEN, CLEAN AND CLEAR OF ANY OBSTRUCTION. SUPERVISOR, NOTIFIED LOCAL AMBULANCE SERVICE TO COME TO LOCATION FOR CARE.
Employee was roof bolting in the face of #8 entry on the #1 unit when a piece of rock, measuring approximately 3ft. long by 2ft wide x by 1 inch thick, fell from the mine roof striking his mid to lower back causing his elbow to strike the panic strip on the roof bolter. The became a Lost Time Accident on August 25, 2011.
Employee was struck by falling steel from the steel rack station giving way.
Electrician/mechanic was change a tire on std shuttle car, when he went to put wedge under tire, the tire fell off the shuttle car hub and landing on Electricians ankle. Electrician continued working on wheel until he completed job then leaving to outside to go home.
EE was stacking bags and was breathing hard, taking in more air than the other hands when he passed out. He came to shortly and was rushed to the town clinic and then transported to the hospital by ambulance where it was confirmed he had high levels of carbon monoxide.
Employee was moving crib blocks from a cross cut. As he lifted a crib block and threw it up on the rib, he felt a pain in his lower abdomen. He continued working for a couple of hours but the pain kept getting stronger.
EE was hanging a curtain when he lost his footing and fell on his left shoulder. He fell against the rib and was complaining of shoulder pain. EE went to the doctor and he continued to work at his regular job until 10/5/11. He had surgery on 10/5/11.
Employee was pulling water hose out of water when he pulled something in his abdomen. Foreman was told but employee said he was okay. His abdomen continued to bother him so he went to the doctor on 9-12-11.
Tractor slid into beam going down slope after being washed down. Employee alleged back pain but felt it was due to farm tractor incident. Medical submitted through insurance. Now alleged as compensation injury. Returned next scheduled shift, worked following week but is presently off work.
Lower back/twisted pelvis using a pin removal tool to remove the equalizer pin from the roller frame the tool followed the pin into the bore. The pin came out and equalizer bar misalignment caused the bar to shift against and side load the tool. The tool handle struck his hand and causing his body to twist putting a strain on lower back.
Worker was walking away from the continuous miner to help repair a burst waterline. A rib approx. 4'6""x 32""x 9"" rolled off striking the victim in the back and knocking him down. Injuries include 2 fractures to pelvis, fractured tailbone and right ankle.
He got a CAT980 loader stuck and jumped from the platform of the loader to the surface rather than using the ladder on the loader. The jump injured his ankle.
Shoveling gravel into holes at quarry shot, hurt shoulder.

Employee states as he kicked against the bottom frame of the belt roller, his right foot was caught by the belt, pinning his leg between the belt and the roller wing. Employee sustained lacerations and abrasions to the back of his lower right leg. IE received medical care and was released for modified duty.
An employee was injured while helping to load a dozer radiator and nose assembly onto a flatbed trailer. As the assembly was being lowered, the crane cable broke causing the assembly to drop several inches and fall forward, pinning the employee against a previously loaded motor. Employee was treated and released from the ER.
Stated that on 8/9 he was stepping over a relay bar and twisted his right knee. EE started missing work on 9/29, to have surgery to repair knee.
Employee was standing on the forks of a loader hanging belt chain in the mine roof for a up coming belt move when he fell to the mine floor.
Employee was hooking up a scoop to a supply skid when he felt a pop (sharp pain) in his right knee. A doctor has scheduled him to take a MRI on 8/19/2011. The extent of the injury will be known after the MRI has been read.
Working on 570 loader replacing u-joint. hammering on the u-joint and a piece of metal broke off and went into employees arm.
The contract labor employee was shoveling coal onto the 16th West conveyor belt. As he threw a shovel load of coal up onto the belt, he mis-judged the height of the belt and his shovel hit the bottom side of the top belt which resulted in the shovel being caught in a top roller. EE injured his right shoulder trying to pull the shovel away from the roller.
While operating dozer, miner struck elbow against arm rest requiring only first aid at this time. Due to lack of improvement of bruised area a return visit to doctor on 9/6/11 required draining of fluid from elbow.
EE states the he was dragging miner cable away from the CM when he tripped over miner cable that was on the ground causing him to fall and injure his lower back.

The contract employee was walking thru the box check stopping just inby the belt portal, As he stepped thru the 5'X4' mandoor and reached back to close the door, the door slammed shut on his left wrist resulting in contusion of the wrist.
Employee was shoveling tailpiece when he tripped over an anchor pin and landed on his left knee.

Dozer operator removing overburden from the detonated shot. Was side-cutting 8' high lift on the left side of the shot, removing material and establishing a tight side-wall pushing on approximate 45 degree angle. He got to the edge of the wall, went to put dozer in reverse when the tight track of machine dipped and rolled.
Employee fell from the second step of a Caterpillar 777 Water Truck resulting in a strained knee. On 8/25/2011, employee was diagnosed with a torn ACL in his left knee. He has been placed off work until surgery on 9/12/2011.
Victim was loading bolting supplies on roof-bolter he turned his body while holding 5 roof bolts and hurt his lower back.
EMPLOYEE WAS RUNNING A CONTINUOUS MINER IN THE #5 HEADING ON 7 SECTION. HE WAS HANGING BACK A LINE CURTAIN WHEN A PIECE OF DRAW ROCK MEASURING 3FT BY 4 FT BY 1 INCH TO PAPER THIN FELL HITTING HIM ON RIGHT KNEE AREA.
Worker was moving MRS unit out of area where mining was completed. He stopped the MRS & walked toward it. When he was appr 8' from front of MRS & about 4' away from ribline a piece of top part of rib broke loose & fell about 6' striking victim on left side of upper torso. He finished working shift. Accident occurred in xcut. Mine ht was about 11 1/2'. He declined med treatment.
Miner noticed an insect bite on his left lower leg when he removed his boot.
Bolting #3 Heading, trying to move gob with head when a pot slipped and fell on his left knee.
Employees had scaled some loose ribs in an old overcast area and was in the process of spot bolting the area. As he was putting the drill steel against the roof a piece of rib fell out knocking employee into the roof bolter controls causing a laceration to his face.
EE states that as he was walking he stepped in a hole and twisted his (R) knee.
He was making repairs to an excavator and was stepping onto the upper portion of an excavator. He lost his footing and fell off of the machine.
Bolting brow when a piece of rock 8"" x 2' x 4' fell out and hit his foot. He was drilling in the rock and it broke loose. Employee twisted back as he moved out of the way.
Employee was backing the roof bolter up to reposition and roof bolt machine came into contact with the employee when it slid to the side, pushing him into the rib.
Employee was putting batteries into a 993k loader when he twisted his knee the wrong way causing his knee to swell and cause pain when he bends it. He didn't go to the doctor until 08/15/2011
Employee was getting out of scoop backwards and stepped in a hole and twisted ankle.
EE was operating scraper #249. He was traveling too fast for conditions and attempted to pass a slower moving scraper. In his attempt to pass the slower scraper, EE hit a hole causing his scraper to bounce hard three times. EE requested that his sup. take him to the E.R. with back pain. ER diag. indicate a compression fracture to the lower back. 1 injured this occurrence.
Employee stated he was working and his left knee started to feel tender.
Employee was standing at the face of a drift when a rock fell from the edge of the wire about the size of a softball striking him on top of the hard hat. It resulted in some muscle/ nerve inflammation.
Injured employee was attempting to lift a trailer coupling that was on the back of wagman hauler. When he felt a pain in the middle of his back.
EE was mounting dozer when he pulled something in his stomach. EE was taken to clinic and given restriction and referred. On 8/15/2011 EE was diagnosed with a hernia and taken off work.
Employee was washing a refuse feed box in the prep plant. He stepped from a concrete floor down to a metal floor approximately 3 inches lower. He slipped and fell striking his left elbow on the floor. He continued to work up until September 1 when he was taken off for surgery to repair a tendon in his left elbow.
While attempting to install rollers on belt employee strained his left shoulder while pulling the roller into position. DID NOT START LOSING TIME UNTIL 8/16/2011.
Employee reported she was opening the bottom of a rail car door when she slipped and fell while attempting to pick up a piece of metal that fell from the rail car onto the knife gate.

The employee was roof bolting in the #3 entry on the MMU. 002 section. The employee was down on his knees and started to stand up when something popped in his right knee.
Stepped off of structural beam left leg first, put pressure on right leg and left leg gave out. Worker was taken to emergency room and diagnosed with torn acl and meniscus.
Oper exiting cab of 5042 D-11 dozer missed step fell to track then ground. Steps and handrails all in good condition. cracked ribs and damaged spleen.
Employee at 1 West Header and slipped going down steps hitting his left elbow on the handrail.
He was using a chisel to trim stone and a piece of the chisel flew off into his wrist. He had no pain when it happened so he didn't report it right away. It started hurting on 8/9/11 and he reported it then.

Employee was loading supplies when a stack roof bolts rolled falling onto employees left leg. Injuries resulted in a fracture to his ankle which required surgery. He will be off work for approximately 6-8 weeks.
EE states that while changing the pot on the roof bolter, a piece of rock fell from the top striking him on his upper back.
EE stated that he was pulling cable off of a shuttle car reel when he felt pain in his left elbow. EE cont'd. to work until 8/10/11 when he was taken off work by a physician.
EE heading up hill-unknown to him the drive coupler tore & truck stopped. Another EE was coming down hill & on CB told him there was a place behind him to drift off the road. Took his foot off brake, truck started to coast backward. Put foot on brake, no brake - no air. Could not steer truck. Panicked & jumped from truck 10' before truck came to rest against berm.
Was taking the safety pan off and let go and it fell on his left arm.
Employee stated that he was sitting in his truck waiting to load. He got up out of his seat to stretch and felt a pain in his stomach.
Employee stepped off the rear step of a mechanic's truck onto unleveled ground, rolling his ankle.
Employee was cleaning coal out of a railroad coal car using a vacuum hose from our vac truck. The vac hose became stuck and while employee was trying to dislodge the hose, his arm got sucked into the hose from the vacuum. This resulted in a strain and contusions in his arm.
Employee was struck by rock that fell from the mine roof while placing the wash down hose on the continuous miner. The rock measured 3' wide by 4' long and up to 3"" thick. Employee was seen on 8/23/2011 by a neurologist at which time he was diagnosed with a back strain and given 5 days away from work.
Employee backed the miner out of place and bumped rib with the miner pushing corner of turning rock loose over top of the coal. The rock struck him on the right side resulting in a broken foot.
Employee was hauling supplies. Piece of draw rock fell from top, striking employee on back of neck and shoulder. Rock measured 36"" long, 6"" to 12"" wide and 1"" to 4"" in thickness. Employee complained of burning sensation in left arm. Rock fell between 2 bolts. Bolts were measured to be 38"" apart in this location.
Employee was standing with his left hand resting on a rib bolt, a rock fell striking his left pinky finger causing a fracture and a laceration which required sutures.
Employee was loading cribs onto ride and twisted lower back-causing pain.
Employee ( coal hauler operator) has been exposed to repeated jarring of his neck while operating his hauler. Finally, after a bouncing occurrence on the date of the report, the employee requested to be taken out of the mine for evaluation. The diagnosis was a neck strain due to the repetitive jarring of the hauler.
Employee stated he was removing a wheel unit from a shuttle car when the ratchet he was using slipped and struck him on the right side of his face causing a fracture and lacerations.
The injury result of the leg on a jackleg drill kicking out which caused the leg handle to strike the drill operator in the knee. The drill operator cont. to work through shift and the following shift at the end of which he was limping and said knee was getting stiff. It was recom. that he stay off his feet and see a Dr. the following Monday if no improvement.
Employee was walking and carrying roller hangers when he tripped and fell, causing his left upper torso to be struck against the hangers he was carrying.
Employee was shoveling 4 head trash pump @ 3 tailpiece when he slipped and struck hisself on back of his head and ear with shovel. Received 7 sutures on his ear.
Employee was walking on cool hill #6 belt. Slipped on mud striking wrist against the structure. Began losing time 8/6/11.
Employee was trying to engage parking break and put his finger in a pinch point near the service break causing a laceration to the right middle finger.
Operator was using the boom of the bolter to dig out the mine floor to gain more height. He did not have the foot jack set so when he raised boom up the canopy dropped striking him on the head, jamming neck. Doctors thought it could possibly be a protruding disc and is put off work as of 8-4-11 during physical therapy.
While truck driver was getting out of his truck he broke door handle of truck and cut his ring finger.
Employee was struck on the left wrist by a piece of mine roof while bolting in 4 West section
Employee was boarding a 336D backhoe, lost his grip and fell backwards to the ground. He had neck and back strain.
The employee was changing screens on the shaker when his glove got hung up on one of the screen hooks, it pulled his hand down on a metal plate and cut his left fourth finger.
Employee was replacing a V-Belt that had come off. He rolled his fingers between the belt and pulley, breaking bones in 3 fingers.
Employee was walking through #4 entry at brk 21 when he stepped in sloppy muddy conditions and fell down. He stated his ankle ""popped"" when he fell. He was transported to surface and taken by ambulance to Summersville Hospital. X-Rays were negative, released with instructions to keep right foot elevated until swelling went away.
Employee was traveling down the 5 block on a decline of approximately 9 percent grade when he lost control of his truck ran off the road to the left up on the embankment and over turned it on its right side.
Closing off the ladder, removed the bolts from the stairs it rolled down impacting the employees left foot.
While another employee was using a hammer to install bucket tooth on loader, employee was watching. A piece of metal came off hammer or pin & struck employee in right eye.
Employee stated that he was getting out of his mechanic's truck. As he stepped onto the ground, he stepped onto a ridge of dirt and sprained his right ankle.
He was performing his regular duties as a beltman when he slipped off a bridge plank and twisted his right knee. He continued to work at his regular duties and continued to have pain. He went to a physician and was removed form work on August 29, 2011 for surgery to repair a torn meniscus.
EE was cleaning under the belt with a ditch which. He then went to scoop another load when the back of the machine lifted and pinched his finger on a bracket. From the bracket to the floor is 54"". From his hand positioning to the floor is 52"". Tow inches of space for his hand placement. The right hand thumb was crushed between the structure and the machine.
EE stated that he put up three rows of bolts, shortly after he felt a numbness in his hand. He does not recall anything specific to have caused this alleged injury.
Employee was climbing down ladder from water truck and slipped. Leg was caught in between the bars and he let go of ladder and fell back and leg was still caught in bars.
The technicians right knee was dislocated/fractured when he was struck by a dragline hoist socket that he was repositioning in a lay down/storage area.
Employee was painting the centerline in the face when a piece of drawrock fell from between the last two rows of bolts striking him on the left shoulder, back and knee.
Miner was operating Cat haul trk when he ran off haul road striking a boulder berm that had been placed to protect trucks from a drainage ditch and culvert pipe. The truck crossed over a boulder hitting a dirt bank causing the operator to hit his head on cab frame
While employee was cleaning out a feeder, he stepped on a wire buried in the mud. The wire penetrated his boot, causing a small cut to his right foot.
As Employee was pulling curtain out of gob he stepped on a rock causing his right knee to twist. Employee went to his family care physician the following day 8-3-11 where xrays were ordered for 8-4-11.
Employee alleged that on 08/02/2011 he was squatting down in a small area and put all his weight on his right foot breaking his right big toe. Employee did not report the accident until 08/08/2011.
Employee was walking to supervisors truck at the asphalt area when he tripped over dozer tracks and hyperexteded his left knee causing swelling in the knee area. Employee was taken to the hospital emergency room where they drained his knee, gave him a pain injection, placed him on crutches and took him of work.
The injured person was assisting a maintenance employee to change a tire. The injured person reached to pick up lug nuts. His foot slipped. He was seen by the mine's Emergency Response Team and then taken to the Emergency room for x-rays where it was determined that there was a broken bone in his ankle.
Employee was getting shuttle car loaded when shuttle bounced and his head hit the canopy of the shuttle car.
Employee was knocking lacing to get back into stope after shooting a brow round. He had knocked 3 lagging and placed them over his joker chute. He tripped and fell in the joker chute. He broke his right leg and had lacerations on his head.
While using a weed whacker employee, who was wearing safety glasses, got a piece of metal in his eye. Abrasion to the eye.
The injured worker was a driller for a geotechnical/environmental engineering company installing monitoring wells for the mine. The worker was attempting to connect a new drill bit to the rig and it fell on his foot, injuring his toe. Steel toed footwear was in use.
EE was working on the suction on his side of a roof bolter. While doing so, his drill steel fell out of the hole he was drilling. His right index finger was mashed between the drill steel and the drill pot. This caused a laceration and fracture to his finger.
Employee was changing a battery on a scoop and while disconnecting the nip, slipped and fell to the mine floor. Lost time injury.
Employee was retrieving slips & wedges from broken cut when block shifted and pinched his finger resulting in a fracture & laceration.
Employee was feeding the steel to the roof. When the steel made contact with the roof the steel bent. Employee grabbed the steel so it wouldn't fall out while letting it down. When employee went to push the feed again he pushed it the wrong way, the steel swung around hitting employee in the wrist.
