A Roof Fall occurred in the 1st East Submain off the Main North at spad number 48+90 in the intersection of #5 entry and extending through the pillarline between #4 and #5 entries measuring approximately 35ft. long x 17.5ft. wide x 8ft. high. The area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts. The area was mined in September 2011.

There has been a fall located in the No.6 entry between break 39 and 40. This fall measured approximately 40' in length X 20' wide X 4' high. The escape way has not been affected. The area of the fall has been dangered and cribbed off. The nearest spad # is 2329.
An unintentional roof fall occurred on Unit #2 in Entry #5 at the 4+20 survey station. The fall occurred due to the strata broke at and above the anchor point. The area had been supported with 4 ft. conventional roof bolts. The fall is approximately 39 ft. long, 19-20 ft. wide, 0-5 ft. high. The fall will be cleaned up and resupported.
A fall of roof occurred in the 4th right panel off the 2nd Southwest Submain, # 22 crosscut, # 3 entry. The fall measured approximately 25-30 feet long x 20 feet wide x 8 feet thick. The area was supported with 5 foot fully grouted roof bolts.
Roof fall was discovered in the #9 seam in the 4th N.E. Submain, #4 entry, between xc-22 and 23. Between spads 9+00 and 9+90. Approx size is 50 feet in length, 19 feet in width, and greater than 6 feet in height.
Spoil side highwall sloughed off due to unstable material (sand and water). The area was accessible at the time of slide. MSHA was called and a 103(j) order was issued. Upon arrival, MSHA inspector modified the initial order and a 103(k) order was issued. Dozers were allowed to slope and stabilize the area and the 103(k) order was terminated.
A roof fall above the anchorage was found in the 4 South intake escape way. The area was dangered off and the escape way was rerouted. The area had been bolted with 5' resin and 12' cable bolts. The fall was 18' X 80' and 6' above the seam.
At approximately 12:25 AM an unplanned fire occurred at the ER Plant, no injuries occurred and all employees were accounted for. The damage is to the processing, flaking, sacking and warehouse areas. The accident investigation is still ongoing so the cause is still undetermined.
A roof fall has occurred in the intersection of Flat Creek 8 #2 entry in the intersection at spad #97+10. The fall measured 35 ft long by 28 ft wide by 5 1/2 ft high.
The hoist car at the Roadfork #51 mine malfunctioned for a period of time that exceeded 30 minutes. When examined it was found that a rectifier had went out and needed replaced. No injuries were reported.
While making return examination discovered roof fall in 5SW butt 4 crosscuts outby face all 5 entries and crosscuts unknown as to length of fall. Section was idle at the time.
A sensor malfunctioned and had to be replaced. Irwin replaced sensor and tested hoist. Placed back in service 9-29-2011
A roof fall was discovered on the 5 North #2 Main Belt. The fall was at the #41 crosscut parallel to the 5 North truck. The fall was approximately 80' in length, 16' in width, and 12' high.
After parking the equipment for the evening, (no night shift at this mine) the security guard called and said one of the dozers was on fire. This was approximately 1 1/2 hours after the shift was completed.
An unplanned roof fall occurred in the Roadfork #51 mine in the #3 entry outby spad #853 that measured 100' long x 20' wide x 6-8' high. This is a continuation of a previous fall. No injuries were reported. The area was developed on 12-16-09.
At 6:13 p.m., miners were being transported on the service cage from underground to the surface for the end of the scheduled shift when the hoist tripped, making the cage stop. A total of 59 employees were confirmed on the cage when the event occurred.
An unplanned roof fall was discovered between the #1 and #2 entry on the 2 south mains. The roof fall measured approximately 80' L X 20' W X 6' T.
A roof fall occurred at 4:30PM on 9/26/11 at xc 15 main south 10 entry 2. Fall was in a four way intersection that was mined in March of 2002 and was supported with 8' full resin bolts. Additional support consisted of truss bolts. No injury resulted water line and power cable was lost. This fall will be cleaned up in future pending approval from MSHA.
An unintentional roof fall occurred on the 5A travel way at station 16+80 in entry #5. The fall did not affect ventilation or cause personal injury. The fall was in the secondary escape way which was re-routed. The fall measured 20'x 25'x 7' and will not be cleaned up.
Occurred in the E-mains section. Section was idle at time. Was discovered on a preshift exam. Fall exists between #2 & #4 entries 2 x-cut out-by face. Was 80' L X 18' W X 6'-8' T . Was boarded with 6' fully grout resign bolts & over bolted 12' cable bolts. Fall area was fully supported & the intersection in #3 entry. #3 was cleaned up to provide a travel way.
A reportable roof fall was found in E South Mains, entry 2, at crosscut 20. The fall was approximately 20' long, 20' wide, and 10' high.
An unintentional roof fall occurred in the 14 I Tailgate entry between 18 x-cut and 19 x-cut. (Spad location 44+60) The fall measured approx 40ft long and 9 ft high by 16ft wide. No injury of equipment damage as a result
No men working. Mine closed, BA status. Mine has been closed down for 27 men thus no men working. 2 foreman make weekly emanations once a week. Fall was found on exam.
An unplanned roof fall occurred in Roadfork #51 mine, 2 breaks outby spad # 388 that measured 40' Long X 20' Wide X 5-7' High. No injuries were reported. The area was developed on 9/2/08.
A possible ignition of methane gas was observed in the No.4 face near the left side of the ripper head. It was immediately extinguished. There were no injuries and no damage to equipment.
Stress built up in ground causing a rockburst. Fractures in back and ribs. Drift and raise obstructed with displaced rock between 3400 level 230/218 intersection to approximately 50' beyond the top of 230 raise.
An unintentional roof fall occurred at the 2nd West headgate Entry #2, 750 West, crosscut 7. The fall was approx. 15' in length, approx. 30' in width and 35' thick. The fall will be cleaned up.
Employee was pushing raw coal on the #1 stacker as he backed up and turned the dozer to the right, the dozer became stuck in the #1 variable feeder. There were no injuries with this occurance
A roof fall was discovered on the Mainline Track parallel to 17 Belt at 12 crosscut. The dimensions of the fall were 25 feet long x 12 feet high x 16' wide.

A non injury roof fall has occurred in the main return on old #19 panel 60 feet inby spad #5747 in the #8 and #9 entries. The was 120 feet long 19 feet wide and height unknown.
A roof fall was found by preshifter during his pre shift run. The roof fall was on #14 beltline 1 cc inby spad # 2521 on 001 section. Fall was approx. 18' to 20' W by 40' Long by 5'-6' thick. No ventilation was affected. No injuries occurred. Fall will be cleaned up and re-bolted.
An unintentional fall occured on HG#5 cross over, #5 entry, between break 9 and 10. The fall measured 19'w x 8'h x 60'l
During inspection of #6 shaft emergency escape hoist, the brakes would not release. Hoist drum was cleaned and brakes were replaced. There were still several means of escape to surface for personnel underground.

A roof fall was discovered in the No 1 return entry of the #2-northwest mains, between break 63 and break 65, just inby survey station #7905. The roof fall was approximately 30'long, by 15' wide and ranged in thickness from 4 1/2' to 5 1/2'. No injuries resulted from the roof fall, and no ventilation effects were detected.
An unintentional roof fall occurred in the #2 x-cut of the 14 I longwall section, the Spad location is 2+70. The Fall measured approx. 23ft wide by 20ft long by 8-10ft high. Not injury or equipment damage.
A roof fall was found during a weekly examination in our M5 1st Left Panel x-cut 21 entry 4-5. The roof fall is located next to the examiners rout of travel. No one was injured, no equipment was damaged, no one trapped, and ventilation was not interrupted.
A NON INJURY ROOF FALL OCCURRED IN THE NO. 9 RETURN ENTRY AT SURVEY STATION NO. 1359. THE FALL IS APPROXIMATELY 18FT. WIDE X 7FT. THICK X 30FT. LONG. THE FALL WAS REPORTED TO MSHA BY TELEPHONE ON 9/20/2011.
A non-injury roof fall accident has occurred in the No 1 entry of the return on the 1st North West Mains. The fall was discovered during the weekly exam of the return air course. The fall is located approximately 10 feet outby survey station No 2318. The fall is approximately 50 feet in length, 20 feet wide and 6 feet thick.
The wana service elevator went down due to damage to the bottom beams and alignment to the counter weight guides. Repairs were made to the elevator and service resumed on 9/22/11 at 6:40 P.M.

An unplanned roof fall occurred in the intake along the #7 belt at break #4 in the #6 entry. 4,000' from the active face. Passage was not affected.
The power was off due to a blowed fuse on the K.U. line side, has to wait on bucket truck from Madisonville, KY to come down. The hoist was down from 10:20 am to 11:25am
The hoist kicked out for a heartbeat fault. The hoist could be resent but when the operator tried to move it the hoist kicked out again. EE came out of the mine to troubleshoot the problem. All controls were reset and connections were cleaned. The hoist was operating again at 1:40am.
There was a roof fall in the intersection on #4 Belt at #40 Break. The fall measured twenty five feet long X four and half feet tall X twelve feet long.
One of the switches on the hoist electrical side fell apart and had to be repaired.

A roof fall measuring 34'long, 16'wide, and 5'high occurred in the #7 Intake entry SE Mains. The fall occurred due to laminated coal and rock above the 5' torque tension roof bolts. No injuries occurred due to the fall. Equipment movement on the active section was affected by the fall.
Weekly examiner located the fall during his exam. He reported it as a continuation of an original fall. During the investigation it was determined that this was not a continuation but a separate fall. This was originally reported as an amendment to E-Docket Number 830168
On 9/19/11 at 10:25 a.m. we had dust and or methane ignition on our 4 right/4 north section in the first x-cut 4-5 room at 0+50.
The primary hoist had blown 2 fuses had to call electrician out to replace and fix the problem
The #1 elevator was out of service for a period of time exceeding 30 minutes due to sch. maintenance. The hoist ropes and govenor rope were changed. The work was performed by Schindler Corp. No individuals ere on the elevator at the time.
An unintentional roof fall has occurred on the 2B-3 Panel in the #3 entry of the 13th XC. The fall was approximately 25 feet in length and 18 feet wide and over 10 feet high.
The primary hoist down due to the computer not talking to the drive system
An unplanned roof fall has occurred at Old Parallel Mains 2 breaks inby spad 21649. The roof fall was approximately 20' x 20' x 5'-6'. No injuries were reported and no equipment was involved.
At approximately 6:35 pm on September 15, 2011 a failure of the Shiendler elevator occurred. A elevator door switch was found to be slightly out of adjustment not allowing the doors to close. The adjustment was made and the elevator was fully functional in less than 1 hour.
There were 13 intersections in the east Longwall bleeder that fell at or above the anchorage zone.
During the weekly examination, a reportable roof fall was found in E10 Headgate, entry 1 to 2, at crosscut 3. The fall was approximately 30' long, 10' high, and 18' wide.
Roof fall in intersection 5A intake 12 blk, area was bolted with 6' resign bolts. #2 ndg: area has been dangered off on all 4 sides.
A roof fall was discovered on the MMU. No. 002-0 section at approximately 7:25pm. The roof fall was reported to MSHA. The roof fall is located in the #4 entry at survey station #3231. The roof fall is approximately 20'feet wide X 20' feet long X 10' feet high. There were no injuries associated with this roof fall.
An unplanned roof fall above the anchorage zone occurred measuring approximately 14ft high x 80ft long x 19ft wide in the right return entry one break inby spad 1876 of #6 a belt. No injuries, entrapment or equipment involved. Ventilation was affected on section no escapeway was affected and travel way was blocked.
Production hoist kicked out and would not reset. Brake wear, brake release, and brake pressure faults for NE1, NE2, and SE2.Jumped out switches to dump skip and put in the middle of the shaft. Adjusted brakes to correct problem.
The weekly examination was being performed when the foreman, EE, found a roof fall in the (A LEFT)panel one break inby spad #552. The foreman timbered all sides of the fall and dangered off the area. Additional timbers were set in nearby entries around the fall.
When testing the over travel switches above the surface landing of the Camp Creek portal elevator/hoist. The elevator/hoist would not re-set to allow the elevator/hoist to travel back to the surface landing.
At 4:45 p.m. on 9/14/11 the site lost power shutting down the fuel system. The system was restarted at 4:46 A.M. on 9/14/11. At 6:05 p.m. the coal in the mill self ignited causing an explosion. The mill was shut down and inspected. No damage to equipment or personnel occurred due to the safety dampers properly venting the combustion gases.
No injuries. Employees were changing bearing on #3 conveyor. Sparks from torch fell to the lowest deck on the scalper screen. Screen deck was rubber and caught fire. Fire was extinguised but started again and and got bigger. The fire department was called to put the fire out.

A roof fall occurred at approx 2:30am 9/14/11. EE 3rd shift mine foreman and his crew was installing belt in the D-mains panel belt line when he heard a roof fall. Upon his investigation he found a roof fall in the A-mining return. He immediately got his crew together and went outside and reported the fall to MSHA
A roof fall occurred on a travelway one crosscut inby station 156. The fall was approximately 60 feet long x 20 feet wide x 10 feet high.

A failure of the Shiendler elevator occurred at approximately 4:00 pm on Sept. 14, 2011. The controller had a control power issue. The problem was found and the elevator was back in operation within 45 minutes.
Fuel Truck # K9079 caught on fire on top of hill at the Halls Creek mine on 2nd shift, operator was o.k. and no one was injured. Calls were made to MSHA and State Agencies informing them of the situation. (See Attached Sheet)
A brush fire was reported between the old Newmont Mill 4 area and the Barrick AA Taildam facility. The fire was in an open field area and impacted a non energized electrical pole line, as well as a company data cable. No injuries occurred as a result of the fire. Fire was contained and extinguished.
A fall of roof has occurred in the South East Submain at 39 crosscut, # 3 entry. The fall measured approximately 20 feet long x 30 feet wide and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
A roof fall measuring 18' long, 18' wide, and 7.5' was discovered 7680' from the active section in the #5 entry inby ss 3195 the old #8 belt entry. Permanent roof supports installed were 4' resin bolts. No injuries occurred from the fall. The fall did not affect ventilation, equipment or the travelway.
A roof fall occurred during the midnight shift in the F-Butt Room set # 5 thru 10 in the #10 room 1 x-cut inby station #3714. The fall measured 4 1/2' high, 20' wide, 25' long and was support with 4' fully grouted rods. No ventilation was impaired and no escapeway was blocked.

An unplanned roof fall has occurred at Horse Creek #4 spad 4022. The roof fall was approximately 40' x 30' x 5'-7'. This area is a weekly travel way. No injuries were report and no equipment was involved.
A roof fall has occurred at 8 mains in the #1 entry at brk. 15, the size of the fall was approx. 20'W x 20'L x 7'-8'H, no equipment effected or ventilation effected.
On the first right panel 002 section #6 belt in the #4 entry a rock fall measuring 1'x17' fell affecting ventilation
A roof fall had occurred on 001 section at 10:00 am in the #4 entry at break 5 outby last open break, the size of the fall was approx. 18'W x 20'L x7'H no ventilation effected, no equipment effected, At 4:07 am 9/14/2011 a fall occurred in #4 entry connecting to previous fall, above the last open break, the fall was approx.18'W x 60'L x 6'H.
An unintentional fall of the roof occurred at the cross cut #95 in the #5 entry at spad #659. This fall is 11,000 ft from the working section. This fall will be cleaned up and supported pending the approval of the clean up plan submitted.
An unintentional roof fall had occurred in the 2 belt break number 9 in the #4 entry. The fall measured 20' X 18' X 4 1/2' thick.
IGNITION - Welding on E-5 belt line at spad 24064 using torch to burn hole in some guarding and the flame was seen between support beam and mine floor. No injuries were incurred.
A roof fall was found and reported to the outside at this mine. The fall had occurred at crosscut #75 between the roadway and the beltline. MSHA 800 hot line was notified at 3:05pm. The confirmation # for the call was 1-88-296882. The dimensions of the fall are 18'wide by 12' to 15'long by 4' to 5' thick. No ventilation interruption or inj occurred.
Roof fall - 30' long X 18' wide X 9' high above the anchorage.
The Shiendler elevator located in the primary escapeway became inoperable at approximately 12:53pm. A control power issue was discovered. Work began immediately to correct the problem.
Contractors were removing an old elevator. A belt was in the bottom and as they cut the bottom section of the elevator out the belt caught fire. The only access to fight the fire was from the top. Could not put out fire in 30 minutes, thus constituting MSHA immediate reportability.
(D-30A) A ROOF FALL was discovered in the #5 entry at survey station 63-68. The fall area was approximately 120' in length, entry wide and 7-8' in thickness. The area was bolted with 42"" conventional type bolts. Timbers were set to prevent access and Heintzman roof support jacks were set to protect the #4 entry.
At approx. 3:50 p.pm smoke was detected coming from a drill (Unit #5025) after mechanics completed repairs on it and left it unattended on the drill bench around 3:30 p.m. Dorton Volunteer Fire Dept. responded and the fire was extinguished by 4:30 p.m.
The bolt on the #2 auxiliary contactor which controls the hoist speed came loose. Miners were evacuated. The contactor bolt was re-secured. The hoist was tested and miners returned underground.
The intake phone would not work so the Foreman traveled the intake to locate the problem thats when he found a roof fall on the 1st East submains in the intake @ spad # 10+50. The fall started in #7 and fell to the edge of #6. We have two intakes. The fall was app 40' long, 18' wide,8-10' high.#7 is not our primary escapeway.
An unintentional fall occurred in the 7+00' x/cut between the #5 & #6 entry's on the 2nd west submain. The fall was 35'L X 19'W X 8' h.
#2 hoist kicked due to a watch dog fault. Fault was reset. There were still several means of escape to surface for personnel underground.
A roof fall occurred in the N01 entry face, of the 001 section, survey spad #2773 +75 feet approximately, above the anchorage of the roofbolts being used. The area was known outcrop and additional roof control measures and precautions were being taken. There were no injuries. The fall did not impede passage or interrupt ventilation in the mine.
The continuous mining machine encountered an unidentified and ungrounded exploration drill hole causing an immediate inundation of water into the face area of the mine's working section. The water inflow ended abruptly once the drill hole had drained, and the face area was pumped clear of the water. There were no injuries to the miners on the working section.
The wana elevator went down at 10:00 am with no persons on board. The elevator was back in service at 10:47 am. Elevator problem with main breaker was the problem.
An unplanned roof fall has occurred on #2 Section at crosscut #3 to #4 spad 5876. The roof fall was approximately 65' x 20' x 6'. No injuries were report and no equipment was involved. A revision to the roof control plan has been submitted to the District Manager for approval.
An ignition of methane occurred near the tailgate of LW-302. The ignition did not propagate out the tailgate entries. Miners at the tailgate applied water to the flame until it had withdrawn into an area behind the face. At that point the mine was evacuated and remains in that state.
Unplanned roof fall occurred at spad # 717 in intersection in #5 entry Old Southeast Panel. The fall was approx. 20' L. X 20' W. X 12' H. No injuries, entrapment or equipment involved. No ventilation or escape way involved.
The cover preventing water from coming into contact with the inspection remote was not in place. This allowed water to contact the electrical components and shut the hoist elevator down. The hoist elevator was down more than 30 minutes.
Roof fall found by fire boss it fell at #10 bk on slope side 4' thick from middle of bk. & feathered out to left rail about 10' inside fall about 20' long total
An unplanned roof fall occurred (extension of old roof) at Spad# 828, 3A belt in #7 entry. Fall was approx. 12'h. X 19' W. X 40'L. No injuries, entrapment or equipment involved. No ventilation ,travel way or escape way involved.
At break 14 #1 belt a rock fall occurred measuring 6'x2' x8"" which knocked a hole in the stopping.
An unintentional roof fall occurred at 30 Break on 1 West Belt. There were no injuries due to the fall. Ventilation or travel was not affected by the fall. The fall measured approximately 50ft long by 20ft wide by 10ft high.
Thirteen employees got trapped on the portal passenger elevator at approximately 12:53 am. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The employees were freed from the elevator at approximately 1:40 am. A defective door switch on the bottom door landing was found to be the problem.
Were retreat mining, had an inexperienced miner operator and was not cleaning the cuts as he was mining. The rock fell on the miner head holding the head on the bottom. The miner could not pull out on it's own due to the fact that the miner had spun the loose coal out from under the cats and the machine saddle bagged on loose coal.
An unintentional roof fall in the return entry of E panel, at 24 X-cut. The fall measured 19' x 100' x 4 to 5' thick.
THE OUT BY FORMAN WAS MAKING THE MAIN RETURNS WHEN HE FOUND A ROOF FALL ONE BRAKE INBY SPAD NO 673 ON THE MUD LICK SIDE. THE FALL WAS 40' LONG BY 19' WIDE BY 5' HI. THE TOP WAS BOLTED WITH 4FT GLUE BOLTS.
A roof fall was discovered in the main east return at crosscut 12-28 in the #6 entry. The fall was in an intersection and above anchorage.
Elevator was shut down during a routine inspection by mine electrician due to the pit at the bottom of the elevator shaft filling with water. The reason for the bottom of shaft filling with water was the pump failing to operate.
Roof Fall 4,500 feet from face #4 entry. Fall was 6' in Height 18' in Width and 50' long. The fall will be cleaned up. Clean up plan is developed.
A roof fall occurred in 1Right 2 South (MMU 002-0) cont. miner section at x-cut 3 between #1 & #2 entries. Fall occurred near the Variable Frequency Drive (VFD) belt controller, but did not strike them. Fall did not impede passage of persons or block an escapeway. Fall measured approx 5'H x 18'W & 18'L. There was a fault near fall. Entire area was bolted w/5'bolts, mats & wire mesh
In the #4 Entry 1 block outby spad #3018, a roof fall occurred that was 18' wide x 18' long x 7' high. The area was dangered off, cribbed on all 4 sides, and cable bolts were installed for 1 block outby and 1 block inby the fall area. The fall will not be cleaned up.
Fall extended above anchorage point, reported to be 8 feet high and the immediate roof appeared to be laminated shale with slickensides. Fall blocked roadway and occurred at No.17 Break of 1Rt.-Southwest Mains, one break inby No.6 Belthead and one break outby spad 1646. Dimensions: 20 feet wide and 23 feel long.
A fall was discovered at Main North #1 Entry between 167-168 block. The fall is between 1219 and 8803, and is 20' long/5'height. No persons affected, ventilation not disrupted, walkway is obstructed. Area dangered off. Face will be bolted and walkway reestablished.
A unplanned roof fall occurred in the #1 entry on the #9 belt line at spad 4423. There is not a producing section at this location. Size of fall 50' L. X 20' W. X 7' H. Ventilations and passage of person was not affected. Fall will not be rehab. Area was initially supported with 60"" Resin Bolts. federal and State Agencies notified immediately.
An unintentional roof fall occurred on South Mains 2, #5 entry, between break 14 & 15. The fall consisted of shale rock and was 30'L x 19'w x 8'h.
The elevator went down at 8:03 PM no one was trapped and the cause was a blown fuse. The elevator was down for 3 hours and 17 minutes.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ALONG THE #4 ENTRY OF 5 SOUTH WEST BUTT WHEN THE MEN WERE TRAVELING TO THE WORKING SECTION
An unplanned roof fall occurred at crosscut #28 of the 1st Southwest Mains, between entry #4 and entry #5. The primary Roof Support was 6' Tension re-bar bolts, and 9' supplemental support in the intersections. The roof fall occurred 9,951' from the #1 working section. The fall was timbered out with cribs built in intersections connected to area.
An unintentional roof fall has occurred in the 2B-3 Panel in the #5 entry in between the 10xc and 11xc. The fall was approximately 30 feet in length and 18 feet wide and approximately 6-7 feet high. Inby and outby this are supplemental support had been added and the fall occurred in-between the support.
A roof fall was found along 2nd South Mains, near station # 5918, in crosscut 4-5. The fall was 4-6 feet high, 30 feet long, and 18 feet wide.
A roof fall measuring 40 feet by 18 feet by 4 thick was discovered in the no. 7 entry at 83 break. Jacks and timbers were set and the fall will not be cleaned up.
At 5:00pm a grass/tree fire entered the mine premises at the north end of the D4 area. The origin and cause of the fire is unknown. The north end of the pit was evacuated at 11:00pm due to reduced visibility from smoke. Local fire departments and Texas Forestry Department were called. The fire was contained at approximately 6:00am on 9/6/11. No injuries or equipment damage.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to the FP relay being tripped. The relay was reset the car was tested and the car was back in at 12 PM. No individuals were on the elevator at the time and all necessary government agencies were notified according to law.
Entrapment of 7 people on the Metz elevator due to power loss.
Unintentional roof fall occurred in the B-1 1RIT 3LIT 1LIT room section. Section was idle when all occurred. Fall occurred in the x-cut 2-1 room 1 x-cut inby sta #8579. The fall was 95' length x 19' wide x 6-8'thick no exscapeways, equipment or people were involved. The area was supported with 4' full grouted roof bolts. All area were timbered and the area abandoned.
A floor scrubber was plugged into a circuit powered by the same breaker for the Fan monitor system. The breaker tripped causing the monitoring system to be out of power for approximately 70 minutes. The breaker was reset, the fan charts marked and the fans were checked to insure that no fans were down during the time the monitoring was interrupted.
A fall of the roof occurred on the 9th 54 x-cut 370 SE LOM Mains C, between the secondary escapeway (supply road) and brattice line. The fall did not block the escapeway. The fall measurements are 20' long by 20' wide by 5' height.
The valve to the east side pinion brake malfunctioned causing the hoist to be down for about two hours.
An unplanned fall of the mine roof occurred above the anchorage zone in the tailgate entry of the 14 left longwall panel at approximately 53+92. The fall did not affect ventilation to the longwall face but did make the tail entry impossible. The fall was a direct result of shearer operator mining above the coal seam.
AN UNPLANNED ROOF FALL OCCURRED AT X-CUT 82 ON THE OLD 6B TRACK AREA. THIS AREA WAS MINED AROUND MARCH 2001. THE FALL WAS ABOUT 60 FOOT LONG, 19 FOOT WIDE, AND 6 TO 8 FOOT HIGH. THE FALL WILL BE CLEAN ACCORDING TO THE APPROVED CLEAN UP PLAN.
On the 2nd day of September, a roof fall occurred outby the sections at S.S. 1851. Separations in the mine roof strata due to layers of lamination is the leading factor to this recent roof fall. The fall measured approx.; 40'Length, 25'Wide, and up to 11' in height.
Two miners were coming down the Alimak Raise when the top motor on the Alimak failed. The Alimak was then stuck in place where it could not move up nor down. Attempts were made unsuccessfully to disengage the motors so that the units controlled free wheel system could be used to come down.
An unintentional roof fall occurred on the Unit 4, 1 North Main West Right Return, Entry 9 Crosscut 51. The roof fall measured approximately 20' in length, 18' in width, and 10' in thickness. A 103 (k) Order (8437817), was issued September 02, 2011, by MSHA Inspector.
Excavator was straddling the trench and tramming back to exit the area when the north side of the trench wall failed. This caused the excavator to fall over and into the trench, damaging the cab and trapping the operator inside for approximately 1.5 hours. Operator was safely evacuated with only minor injuries.
Roof fall occurred along 14 belt intake at 15 brk area was developed approx 6-14-2010. The size of fall was 18'l x 18'w x 10'-12' high laminated sandstone. Area was bolted with 6' tension resin bolts and supplemented with 12' cable bolts
A roof fall was discovered at 94-95 Brk #2 entry of 1 South mains. This fall does not impair travel or ventilation. The fall measured approximately 110'L x 20'W x 5'T.
There was a roof fall at #6 entry, 117 break of the West Mains bleeder system. The fall was in a worked out area, will not be cleaned up, and was found by an examiner.
A dozer operator making shift change encountered a hard spot in a high wall. The dozer became slightly offset with the highwall lost power and could not back up the slope. The dozer operator was assisted up the slope with no injuries. He was in the dozer less than 10 minutes. This 7000-1 is filed in protest arising from a citation issued for failing to report under 50.10(c).
A roof fall measuring 30 long, 18 wide, 6 thick was discovered on the SE Mains 1 crosscut inby spad 66710 #5 entry due to coal and rock streaks above the anchorage zone. The fall was 4,400 from the section, mine operations, ventilation, and the passage of men was not affected. The area was developed with 5 torque tension roof bolts was the primary support.
Roof fall 2 breaks inby spad #235. No injury or entrapment. Old Works.
SC511 Screensizer chute plugged with hot material which emanates gases that combust when exposed to air. Flames escape through doors located on the top of the screensizer. No damage to equipment or personnel. Equipment is designed to withstand this condition.
A roof fall was reported at 2:52 PM, along 2nd East Mains, in crosscut 4-5. The fall was approximately 42 inches high, 18 feet wide, 30 feet long.
Employee was changing screen material in a triple deck screen when he fell over the hand rail. He fell approximately 56 feet to the ground below.
Co-worker was in the process of shutting down the generators, to do maintenance to the power cables. At that time employee started to unguard the power cables on the switch van out of the vision of co-worker. He refused to have the power checked by two other co-workers. Moments later he was electrocuted.
The investigation is not complete, but as of now it appears a miner was fatally injured when material that traveled through a borehole struck him after a blast was initiated. The victim was on the 1290 level ramp waiting for a blast to be initiated. The concussion from the explosion traveled through a 3-inch borehole, blowing small rock and debris onto the victim.
Drill crew was attempting to back off a stuck drill pipe. A wire rope snub line was used to secure the worm tong. The snub line failed and the tong struck the employee resulting in fatal injuries.
EE's right hand became caught between a chain and sprocket resulting in loss of three fingers.
Employee was removing a rock from the #2 take-up when he felt a pain in his shoulder, back and abdomen. He continued to work until 10-3-11 when he started missing work. Workers Comp was looki ng to see if a pre-existing condition existed and the Safety Dept. was notified on 11-2-11 that the accident did not involve a pre-existing condition. Diagnosed with a hernia.
He was traveling toward the face when a piece of rock fell from the mine roof striking his lower left leg. This resulted in a compound fracture. He was treated and transported to the hospital. Surgery was performed on the injured employee leg immediately, however due to the complications with the arteries, later his leg was amputated below the knee.
The employee was attempting to remove a crane from a mechanics truck when the two chains he was using one of them came off. This allowed the base of the crane to fall to the floor. The employee was then struck in the leg by the crane and received a compound fracture to the right leg.
Employee lifted the right fork lift (on a Terex forklift) in an attempt to reposition it to gain clearance to the compressor enclosure for the purpose of compressor motor replacement. Vertical portion of fork crushed tip of employees' s finger when fork release to running position. EE misjudged position of hand relative to fork.
Employee was replacing an access door on a vertical screw when his glove got caught by the screw. The tip of his left index finger was severed.
In the process of unhooking a trailer mounted generator from behind a pick-up truck, the employee was attempting to loosen the coupler from the truck. While attempting to remove the coupler, the employee attempted to lift the tongue and felt a burning pain in his stomach. A bulge later appeared on the top of his stomach.
While changing out an equalizer bar on a D10R Track Dozer, an employee had his hand in a pinch point between the equalizer bar and Dozer frame while he was using a wrench. The employee's left pinky and ring finger were pinched between the frame and equalizer bar resulting in a crushing/amputation injury to these two fingers.
Employee was pulling draw rock when a piece struck his hand.
The operator ran the loader off the right side of the road onto the bank and turned over in middle of the road.
EE was changing a boom jack on a fletcher bolt machine when he lost his grip on the jack. Jack fell on finger catching it between jack and frame of bolt machine, injury caused laceration on left hand ring finger,(8) eight stitches were required to close laceration.
Employee called into work on October 3rd, stating that he had hurt his foot and would not be coming in. On 10/11 he called his boss and stated that he had to go for additional testing to confirm if his injury may be related to work. On 10/17/2011 he was seen by the Workmen's comp physician who ordered and MRI. On 10/20/2011 results of MRI confirm work related injury.
Slipped getting out of 602 Scoop and straddled seat and frame scrape in groin area.
Man was installing a roof bolt after drilling the hole and got his pointer finger on his right hand caught between the wrench and the washer on the bolt, he missed the hole and put pressure on the bolt against the roof.
While checking the radiator of a bobcat loader employee's right ring and middle fingers came in contact with fan blades resulting laceration and fracture to both fingers.
The I.E.stated that while hand dusting the roof, some of the dust blew back into his eye.
When leaving work area at end of shift he hit his head on a roof bolt jamming his neck.
The victim was operating a rock truck. When the shovel was loading the truck, a rock shook the truck and jarred the operator. He began experiencing pain in his lower back and groin area.
While operating a shuttle car, ee got off the car to move the continuous miner cable out of the haul road. While lifting and pulling, he strained his left shoulder and neck. He came back to work the following day, but was unable to complete the shift. He went to the ER the next day and was told to stay off work until further tests could be made.
Employee was mounting #2527 rock truck placing right foot on step to pull himself up and felt pain in his calf of his right leg.Employee was taken to UAB West emergency room evaluated and released.Referred to Dr. and released to full duty on 10/05/11
EMPLOYEE WAS WORKING ON THE TRACTOR AND AS HE WENT TO GET DOWN OFF IT, HIS PANT LEG CAUGHT THE CLUTCH PEDDLE CAUSING HIM TO LOSE BALANCE AND FALL. HIS LEG HIT THE FRAME OF THE TRACTOR CAUSING A SMALL FRACTURE TO HIS LOWER RIGHT LEG
Employee was lowering a re-lay bar with a pair of come-a-longs when the chain on the come-a-long broke and hit employee in the right hand.
Repeated neck and back snapping from sudden stops when hitting unseen immovable objects while digging with a front end loader.
Employee was shoveling under the crusher when ""he felt something give"" in his shoulder.
Muscles in the lower back are sore from splitting stone
Employee was lining in the Primary Mill. He was using an alignment bar to position the liner in place, the liner popped up and the alignment bar struck the employee on the left side. He was evaluated by a Physician and released back to work. The employee missed one shift due to medication administration. He returned to work after missing one shift.
Employee was walking through crosscut from belt to roadway, up an incline, when he slipped and fell on his right side, pulling muscle in his right leg.
Driver stepped into cement dust that overflowed his work boots.
The employee attempted to remove a rock from beneath the lifeline. The rock was approximately 24"" x 12"" x 12"" and weighed approximately 50 lbs. After lifting the rock, the employee felt a pain in his back. He finished his shift and continued to report to work as scheduled until 10/11/2011, which was his first shift missed.
While trying to pull the drive on site, the chain snapped and the come along hook penetrated the left arm.
Believe accident happened sometime in the afternoon. Employee believes when he was climbing ladder @ plant site, some of the rust/metal from the ladder fell into his eye. Employee was climbing ladder to clean screens at plant site. Employee was wearing safety glasses, however admits they were too big.
Employee was working on a dozer, taking oil plug out of final drive. He was jerking on a wrench to remove the plug when his back popped and he fell to the ground in pain. This accident is reportable because the employee missed the rest of that day and the following day.
While getting a rock sample, he climbed out of the bunker with the full bucket, lifted the bucket over his head to the railing and slipped on the side wall of the bunker with his arms extended out. He felt the muscles in his back stretch.
The driver was attempting to straighten a bent hand rail to dismount truck, when the hand rail broke, causing him to fall head first to the pit floor. The driver sustained simple, compound, and hairline fractures, and cuts and abrasions.
Employee states he was trying to unclog the hopper on a duster. As he reached inside the auger housing, a co-worker activated the hydraulic control, causing IE's middle finger to get caught by the auger. EE sustained a fracture and finger-tip avulsion of the middle finger.
The IE and the hauler operators were attempting to extend the ventilation tube slider, when the roof collapsed. The falling roof hit the tubing which knocked the IE down and then the rock hit the EE on the left foot. The falling material cause laceration to back and laceration and crushing fractures to his lt. foot. Rock size 1' - 3' thick and approx. 20' L. X 8' W.
Employee was found outside of the mine laying on the backside of the Slope Belt. His last known work location was around the Main South Belt Tailpiece area. His only memory of the accident is being on top of the Main South Belt and striking the top of his head on a roof bolt. The cause of the accident is unknown.
Walking through curtain at end of shift, hit bucket of scoop and rolled left ankle.
Miner operator and two shuttle car operators were hanging the miner cable. The miner operator was on his knees and over-stretched while lifting the cable. He complained of his back hurting. He was transported to CAMC where he was diagnosed with a muscle strain in his back. He was given two days off.
Employee was installing water line on belt structure when a pressurized hose struck his legs. Employee suffered bruises and swelling to his legs resulting in lost work days.
While dismounting from a highwall drill, the EE injured his ankle contacting the ground, X-rays show it to be fractured.
Employee was installing a pin (7""x20"", 210 lbs) into a 2570 drag chain clevice.The pin was installed thru the top hole and came to rest against the flange of the bottom hole. Employee removed the hoist strap and tried to turn the pin by hand. The pin slipped into the bottom hole, pinching his finger between the pin top and side of clevice.
Employee picked up top roller off manbus and felt pain in back. Employee has been working up to date 10/31/11. TAKEN OFF DUE TO SURGERY.
Recoil on seat on ET-2102 bad causing a strain on lower back. Did not go to hospital until 9/28/11.
Employee was walking beltline when he twisted his ankle due to muddy conditions. He was transported to surface and transported by ambulance to Greenbrier Med. Center. x-rays reveled a fracture to left ankle
At approximately 11:30 am ee received an electrical shock. EE was standing on the second step up from the ground of a 6' aluminum ladder which was leaning against a conveyor belts metal tail pulley guarding. He was adjusting a nearby water valve with his right hand as he was holding the guarding with his left hand.
EE was pushing/installing a 16 foot cable roof bolt when he felt pain in his right shoulder.
EE was helping change a hydraulic hose on the roof bolter when a rib rolled off and struck him in the hip and leg area. EE was treated and the mine and transported to Middlesboro ARH however he was airlifted to the University of Tennessee hospital rather than MARH. He had surgery for a fracture in the hip and he also had a compression fracture of the L2 vertebrae.
Using a flashlight to approach trailer where gas was stored. Lost footing in dark area, tripped, struck arm against trailer fracturing it.
Injured was prying rock out of shuttle car conv. boom when slate bar caught left hand between shuttle car and slate bar. *Started missing work on 10/10/2011.
Employee was washing the slope belt when he went to turn off water and lost control of the hose with it striking his lower left leg.
Employee was splicing wire for CB radio on a 924 Loader when his knife slipped, and sliced his right thumb. Employee went to doctor on 9/27/2011.
He was working as a shearer operator, he stepped into the panline to remove wire from the tailgate drum and twisted his right knee. *We dispute the allegations in Citation #8052781 and submit this report solely to terminate the citation.
EE was removing a chock block from the holder mounted on the front of a Cat 789 truck (unit 706) when the chock slipped out of their hand and struck the left leg above the knee.
Employee stated that while attempting to dismount #12 motor at 14 Left switch he missed the step on the outside of the motor and landed awkwardly on his left foot, resulting in a fracture to his lower leg and ankle.
Working on a belt drive installation. He was welding on a belt drive installation guarding when a piece of rock struck him on the right hand causing a fracture of the little finger.
Employee was emptying the baghouse hopper. The dust from it went up pant leg.
While gathering tools to begin job, the involved employee lifted a roll of fence wire and experienced pain in his lower back.
Pulling center line out. Had weight tied to it. Weight fell off and down shaft, striking employee on top of head. Had hard hat on. Causing laceration to top of head
Employee was shoveling at the beltline when a rock fell from the roof and struck him on his head. Employee was taken to Vincennes ER where he was ordered to remain off work for an uncertain period of time.
Miner was attempting to dislodge a jammed split set from the bolter using a scaling bar. When the split set popped up and out, it struck the miner on the left side of his chest resulting in a contusion.
The miner operator was repositioning the miner in the #8 entry on 002 section. As he was setting the miner over, a piece of rib fell out and struck him on his right leg. Employee was attended to by to M.E.T. on the section and transported outside. Employee has a possible fracture to right leg and ankle.
Employee was lifting belt roller from jeep and roller twisted causing his wrist to turn in to body and injured right wrist. Started losing time 10/13/11.
Employee was bolting the back on the Foot Wall side when a small rock fell striking him in the cheek, causing a laceration. sutures required. days missed
EE states that as he was getting into a scooter he felt/heard a ""pop"" in his right knee. Due to circumstances associated with this event, the mine operator is ""Red Lining"" this event as there may prove to be a non-occupational injury. Incident is under investigation. EE started losing work days on 10/17/2011,Per call11/22/11 with A.C. change missing date to 09/30/11.
He was hanging the miner cable and felt pain in right shoulder. He worked full time until the MRI showed a small tear in shoulder. He started missing work on 2-13-2012.
EE was lifting belt while making a belt move. He lifted the belt and turned and pulled a muscle in his back.
Employee was setting a water pump unit #1589 with a utility loader. The pump needed jumped. When he hooked up the jumper cables a 3 inch flame started on the cable on loader. When employee went to get fire ext. he stepped in a hole in mud and twisted his right knee.
EE was removing cap from air line and failed to completely turn off valve allowing air pressure to be under cap. When cap was removed it struck EE on hand. EE was operating Cat 994 endloader.
F-22 HG section, track entry, 3 1/2 to 4 Cross cut, employee was helping to spot a track rail when the rail slid off the scoop, bounced, striking him below the left knee. Left Patella avulsion fracture- left patella tendon rupture.
Employee was working on bulk lime tank installing conduit for a level detector. While climbing the ladder he felt a pain in his left shoulder. After follow-up doctor's visit the employee was place on restricted duty to begin on 10/24/2011.
Employee stepped off the back of a rock dust car and twisted his left foot.
He was retrieving a come-along off of the discharge end of the stage loader, when a rock passed through the stage loader causing the cover to raise, which struck him in the face.
A rock measuring 4' x 7' x 1""-4"" fell striking Employee on his right foot. Employee was transported to Raleigh General Hospital and was diagnosed with a fractured big toe. Employee was scheduled for out patient surgery but instead was admitted on 9-23-11 for surgery to correct his fractured toe.
Employee states that as he was checking the Nellis dump, he lost his footing and fell approximately 25 foot over the dump. The dump area was at the angle of repose. Weather conditions were dark and wet.
The employee bent over to pick up and get out the way a section of miner water line. When he bent over he stated that he felt a pop in his back.
As Employee was bolting in #7 entry the drill chuck was not lined up with the bolt head. Employee hit the rotation lever causing his hand to twist in the bolt. Employee was treated at the scene by EMTs and drove himself to Raleigh General Hospital for further evaluation.
A contract employee (J & H Enterprises - I.D. # D671) was operating a Mack coal hauler on the Plant haul road when he lost control of the truck, struck a berm, and flipped the truck over onto the roadway. He suffered lacerations to the head and a concussion. The driver missed the next working day and returned to work on Monday September 26th.
While pulling supplies a pin broke allowing the Wilson coupling to fall on the ground. The employee picked up the coupling and carried it back to the trailer. When he lifted it again he strained his back. He finished his shift but started missing the next day.
While trying to remove a screw from the floorboard of a backhoe, the employee felt a pull to the right shoulder. Felt pain that worsened over the remainder of the shift, was seen at ER and orthopedist, diagnosed with a partial rotator cuff tear and sent to physical therapy.
The employee was struck on top of left foot by a rub rail on the continuous miner upon removal of hold hinge pin. The incident resulted in a Fx metatarsal bone.
Employee was dislodging a wedged rock from between the rear tire and the cab of loader and felt a pop in several places in his right forearm. Strain to right forearm.
Injured worker was stepping across to a beam. A rock rolled under his grounded foot, causing his leg to bend the wrong way and pop.
Employee was assisting in bolting on a fall at 17-12. The drill operator pushed the bolt up with the drill, the bolt bent and kicked the drill sideways pinching his finger between the bolt and volcano plate.
Employee was drilling a hole into the mine roof to install a roof bolt. A piece of draw rock fell striking him on his right hand. Infection set up into his hand/arm. He started missing work on 9-26-2011.
While placing an impact gun on a table with his left hand, employee states that he felt a ""pop"" in his left shoulder & then an intense pain. Suspected tear. He is not to use his left arm/shoulder. We cannot accommodate these restrictions, therefore this is now a lost time incident.
Tightening a bolt with wrench hand slipped and hit thumb on top flange of the beam. Few days later it got infected.
While moving and hanging the continuous miner cable and water line during the normal mining cycle, employee strained his left shoulder. He continued to work until the pain reached intolerable levels. Later an MRI showed a tear, and began missing work on 01-27-2012.
EE was moving a set of cable crossovers. He felt pain in his lower back and the left side of his middle and upper back
Employee was walking when he tripped causing him to fall, the employee fractured his left wrist in attempt to catch his fall.
Employee was pulling grease line from service truck #1301 to grease a rock truck. the employee turned/twisted and felt pain in lower back. Employee was taken to UAB emergence room for evaluation. Employee was released and told to see his family DR. and could report back to work on 9/26/11. Employee had previous lumbar strain. Reel on truck worked properly.
Employee was drilling 60"" hole to install roof bolt when a loose piece of roof fell, striking his left palm.
Injured was backing continuous miner out of a cut when he was struck by a piece of roof rock that had dislodged from between the bolts in the intersection resulting in fractured bones in his right foot.
Injured was sitting beside bolter when roof straps slipped off bolter striking left hand.
Employee was standing between two 6 1/2 foot omega cans that were lying on the mine floor and as the can setter operator went to retrieve another can, the cans were rolled together thus smashing both of the employee's leg.
Employee was standing along side of a shuttle car looking into the cable reel compartment of the car when the rib sloughed off and the employee was hit in the back of the right knee by a piece of coal.
Employee was climbing up a ladder onto a clinker rail car when he went to sit down on the rail car catwalk and twisted his right knee.
While hauling water line employee's finger was mashed
EE (Roof Bolter Operator) was preparing to install roof bolts when a roof bolt fell off the machine and lodged behind an operating lever, causing the machine to swing over against EE, which caused bruising and pain to right side, rib cage area.
The employee reports he injured his lower back disconnecting a winch rope from a trash bin. He claims he was standing in mud and flipped the rope hook backwards creating a lost time injury. His medical suggest he has history of same said problems reported in the past.
Employee was unsure when suspected injury occurred, may have pulled back when lifting barrier beam or getting up and down when cutting hezman jacks. Employee stated that he did feel something when he lifted barrier beam, but thought it might not be nothing as shift went on, stated he was feeling pain.
EE was operating roof the bolter on the operator side, standing on ground. The operator man-up platform, which was chained up, fell striking his left knee.
Walking across yard and had sharp pain in back and leg and legs buckled.
Associate with pre-existing back condition bent over from a sitting position to pick up a piece of paper and strained her back.
Employee was helping to unload a bundle of T5 roof straps when they started to slip. He took off running and twisted his knee due to muddy conditions in the area.
While walking up a ramp during wet conditions the employee slipped resulting in his left knee being twisted. The employee had walked down the ramp in order to be able to assist in fueling a pump.
He was bending over along the slope belt installing water line when an object came between the skirt rubber and the belt and struck him in the head. He was transported to the hospital and kept over night for observation. He is diagnosed with a closed head injury with concussion.
The employee in reference has allegedly stated that his lower back & right leg has been injured while working at the facility crusher run tunnel area. The employee was operating a D8-R cat dozer in & around the crusher run tunnel stockpile area at the time of the incident. The employee allegedly has stopped the dozer & stepped down onto the stockpile area to relieve his self.
The injured and three others were going to recover some high voltage cable from old works, they had knocked an old stopping to gain access to the area, after they traveled thru the old stopping with a scoop they were attempting to hang a piece of curtain up and some block that was left on the roof had fell and struck the employee in the back of the head.
He was changing oil in PC450 Komatsu Excavator. He was standing beside engine when he went to pull down hood to close, hand slipped off causing him to fall backwards into ringer oil container, causing pain to his lower back.
Roof bolter was drilling a hole for roof bolt the drill steel broke and the employee went to pull the steel out of the hole, his body came in contact with the machine causing the drill steel to rotate catching his hand twisting it causing injury to middle finger.

Individual was walking along the kulntown rockdust chute when the left foot became caught in a tie down rope causing the individual to trip. The individual fell with the right hand on rail and the left hand on the ground. Individual went off work for surgery to the shoulder - rotator cuff 1/5/12.

Miner was struck by 13 wide x 53 long x 3/16 thick steel access plate as he exited area below Mesabi vibrating screen. MRI showed normal and unremarkable. Diagnosis: neck pain similar to whiplash. Second opinion: cervical strain (sore neck).

Employee was carrying a piece of belt structure when he stepped not a hole, falling and began to feel pain in his ankle.
Employee was walking in # 5 entry and felt pain under his left patella. Employee stated that he had knee problems previously. Company received medical documentation that made the injury a reportable. Employee had a tear of the meniscus of the left knee.
End of shift employee was getting out of Volvo Rock Truck, coming down steps, holding on to hand rail, thought ground was close and jumped backwards while hanging onto the hand rail injuring right shoulder.
EE claims he slipped on wet material while attempting to fill a 5 gallon pail with lube in order to start his lube duties. EE was seen limping into work and this accident is under investigation for validity.

Employee was working bent over. When he raised up, he felt a pain in his back.
Foreman was operating 992 when a hydraulic line had blown which sprayed hyd fluid on windshield & surrounding area on eq. EE exited the loader & attempted to clean the windshield. When he went to move to the fender or boom area, he apparently slipped on the hyd fluid and fell from the loader onto the ground causing injury to his left side.
Employee was working underneath shuttle car making repairs to broken conveyor chain. As he was crawling under the car he dislocated his right knee. Employee finished his shift and sought medical treatment the next day. Was instructed not to return to work until seen by a orthopedic surgeon.
Employee was sweeping the walkway along main #7 belt at 45 crosscut. Tripped over spotted roller on the mine floor and rolled his right ankle. On 9-18-11 was diagnosed with a fracture of bone.
Employee changing screen on plant reached for screen caught finger between screen and safety rail. Received 3 stitches.
Employee was feeding the plant with the loader. The feed was hard to work with -lots of stick, wet, roots - didn't want to fall. A lot of banging around and twisting and turning which caused the employee lower back pain; he can feel it popping when he moves around.
EE's coat sleeve got caught on drill steel, twisting his arm. His partner reversed controls to untwist the arm. Damage appeared to be a contusion (no broken skin or bones),treated with 1st Aid, but in evening, EE decided to go to the doctor. X-rays proved no broken bones, but the doctor decided to drain fluid and observe him overnight monitoring for muscle damage.
While installing roof support, a piece of rock fell from the mine roof striking him on the lower back.

Operator of the RTD exited the cab at the Southside Queue. As he approached the top of the stairs on the rubber tire, he felt his left knee pop and felt pain in the knee. He was taken to his physician, given a brace and crutches and an MRI was scheduled. Taken off work and currently ONGOING.

Employee first reported a groin strain 9/16/11 but did not need medical attention at that time and continued to work. EE developed a bump which was found to be a hernia and went off work April 9, 2012 for surgery, This will be a lost time occurred.
Employee alleges he was driving loader to the crusher to haul rock and the loader jerked causing him to twist his lower back. We were just notified on 11-08-11 that he was going to be receiving his first epidural steroid injection. Depending on how he feels he could receive more. The employee is not loosing any time.

While employee was setting cap on crusher, he was lining up the bolts on the plate. While he was holding a nut for placement, the bolt fell and crushed the pinky finger on his right hand. Finger was lacerated and seven (7) stitches were administered.
EE was assisting in cleaning up material with a vacuum truck. He had attached a rope to the end of the hose and wrapped the other end around his hand. When the hose moved it pulled the rope attached to his arm and dislocated his shoulder.
Employee was riding in a man trip when it hit a rib causing him to hit his nose and fore head.
When exiting 190 New Holland skid steer foot slipped off step and landed on ground.

Emp tripped while going thru a revolving frame of an 8750 drag line injuring his right bicep muscle. Emp was taken to ER where the doctor diagnosed him with a bicep rupture. Emp did receive a full return to work and was referred by the ER doctor to a specialist on 9/15. The specialist required a MRI on 9/20. On 9/26, emp received results of MRI indicating that surgery was needed.

Electrician was replacing a battery plug when a rock measuring 5'x3'x3 1/2"" fell from the roof striking him on the back of head & neck area. He drove himself to Raleigh General and the doctors gave him a full release. He returned to work & continued to work until 9/27/2011. He went to his doctor who gave him two days off with muscle relaxers. He returned to full duty on 9/30/2011.
Employee was setting a crib. Was swinging a sledge hammer to tighten a crib. Lost his balance and struck his Lt index finger between the crib and sledge hammer.
Pulling table bushing out of a drill and aggravated his hernia from July, 2011. Went to see a doctor on September 27, 2011.
Employee, a motorman was stepping onto a motor when he lost his balance. He tried to shift his weight to catch himself and twisted his knee. He said he felt it pop. He was brought outside and transported via ambulance to the Princeton ER. He was given time off until 9/19/2011 by the ER doctor, and has a referral with an ortho specialist.
Employee was operating a dozer and while backing up he hit a rock and jarred his neck.
Individual was a rib bolter on the 18w section fall #1 entry. The corner of coal pic rolled out striking the right side of face, pinned both legs thighs and right ankle. Individual suffered an abrasion to the right temple area, and bruises to the thigh area.
While hooking a chain between a scoop and a disabled ram car had his right foot pinched when the ram car moved. He was transported by ambulance where X-rays revealed no fractures yet the removal of damaged tissue and the suturing of lacerations caused this to be a lost time injury.
Employee was carrying a 5 gal bucket of oil when his foot slipped and he felt pain in his lower back.
Employee was walking alone, back to the beltline to continue splicing when a rock fell from the roof and struck him on his head. No visible signs of injury but complained of pain through neck, back and feet. Employee was taken to Vincennes ER. Employee was ordered to remain off work for an uncertain period of time.
Employee stated that he was hanging a temporary curtain in rear window of loader so as to block glare from the light plant lights. His hand hit the overhead liner on the cab causing dust/dirt particle to fall into left eye.
Employee was lifting a bundle of wood stakes to put into the back of the pickup. While doing so, the bundle of wood stakes hit the buggy whip causing the wood stakes to slip away from his hands. Employee tried to catch the wood stakes and twisted his upper body.
Walking to the loader, he stepped on a rock and twisted his left knee. Original RPI on 9/14/2011. He became lost time on 10/14/2011.
End of shift crew headed to the Mill bench, crew walked up to building not seeing bear. Bear charged crew, running into EE. Bear threw him into the door jams and to ground. Bear then ran off.
EE was working on the number one dryer replacing the thrust idler. He was repositioning the old idler on the base for removal when he felt pain in his left shoulder. He was not using excessive force in the move because graphite was on the base acting as a lubricant allowing it to slide easily. The dryer was locked/tagged out and was jacked up hill and blocked against motion.
The miner was loading top structure in a trailer in preparation for the next belt set-up. When placing a piece of top structure on the trailer, he felt a sharp pain in his right shoulder.
Employee was taking out belt roller and felt something pop in lower back.

Employee was pulling on a ventilation curtain that was covered with mud located between #4 and #5 crosscut last open break on MMU 009. He felt pain in his lower back.

Employee was operating 988B Loader. Exhaust leak developed with fumes entering operator's cab. Employee became nauseous and light-headed and had to leave work.
The employee stated he bounced inside the operator's compartment and contact the seat with his tail bone causing an injury. The event has resulted in a lost time injury.

Miner was removing a bearing race from shaft. He had cut the race with a Metabo. After the cut was made, he tried to remove the race by striking with a hammer. At that time a small piece of the race broke off and punctured his lower left forearm. He was wearing a hardhat with safety shield attached at the time.
Employee was shoveling around belt tailpiece when a cable tender chain broke and struck him on the right side of his body in the rib area.

Injured employee was struck on the upper back and neck by a rock weighing approx. 6.3lbs that fell approx. 70 feet from above from the RC4 belt. Injured employee was treated by EMT's and EMS then airlifted to CAMC-General ED. He was admitted for observation and diagnosed with vertebrae fractures of the neck and tissue bruising.

The contract employee entered the mine at 9:00 a.m. He stated to two co-workers that he had injured his back over the weekend. At 1:05 p.m. he called to the mine surface and alleged to the mine superintendent that he had injured his back at the #5 belt drive. Abby Contractors is controverting the workers compensation claim.
Getting off Volvo #1 A40 and as he climbed down the steps he slipped off and struck the side of the vehicle causing contusions to his left shoulder, right leg, and head.
Mine Mechanic was preparing to fill air compressor with gas. Lid on 5 gallon gas can was ajar, causing gas to spill and vapors to be released. Nozzle contacted battery cable, which sparked and ignited gas on Mechanic's shirt, causing burns to arm, hand, neck and chest.
Employee was alone, switching rides when he stepped out of mantrip and his back ""Popped"". Employee worked the rest of shift, didn't report accident to foreman and worked the next couple days. Employee called in on the 13th and reported accident and came in on the 14th to file report. Went to Dr. where he was ordered to stay off work for an uncertain period of time.
Miner was operating a Cat 992C loader and felt a burning sensation in right forearm. Visit to med. provider on 09-12-2011 diagnosed infected insect bite. Followed up on 09-14-2011 and admitted to hospital to begin treatment of Staph infected insect bite.
Third shift crew was replacing a bad pump located in the creek on the surface. The crew foreman strained his back and later sought medical attention. He was given medications and taken off work. He missed 3 days work during the following week. He RTW on 9/19/11 and has missed no more work. The safety department learned of this accident on 9/19/11 and began investigating.
EE was placing a roof channel on the timber jack of the miner and caught his right index finger in between the timber jack and roof channel causing a laceration to his right index finger.
Employee ALLEGES the mantrip went dead at the top of hill. It rolled back, he pushed the brake pedal down but it busted. He then set the park brake but it didn't work. The rear end of the mantrip went into the rib. The employee complained his lower back was hurt. We DON'T believe the ALLEGED accident occurred due to defective equipment but rather due to the brake lines being cut!
Contractor setting up equipment. stepped off ladder from drill and stepped on a stone twisting his ankle and braking a bone in his left foot.

The crew was moving the miner from #5 to #3. The buggy men were assisting with the miner cable. EE bent over to move the cable when a rock fell from between bolts knocking his hat off. A second rock fell striking him on the back of the head. He fell to the floor and was administered care after being moved from the spot where it happened.

Victim was stepping onto the bottom rung of the ladder to climb onto the Caterpillar 777 he was operating when his foot slipped. He sustained an avulsion fracture to his right ankle. He took himself to the hospital. They x-rayed, and splinted his ankle, he was released and returned to work on the 14th with no restrictions. Missed 3 days work.
Traveling through an access hole cut in a back-up check and foot caught on canvas. He fell and fractured his left fibula.

Employee had climbed up the ladder to check fuel level in fuel truck. When he was coming down his foot slipped and he grabbed for the hand rail but missed it. Then he fell to the ground, about 4 feet, he fell on his back. (Bruised lower back)
Employee was changing hose on roof bolter when a piece of rock fell onto his back, knocking him into the bolter. He continued to work his scheduled shifts until 2/3/12 when he started missing work.
Supervisor was sitting in parked vehicle, when he was struck by employee personal vehicle head on going about 15-20 mph at the time of incident. Supervisor sustained injury to back.
Employee was welding inside a belt head rock box. Subjected to reflective flash off metal plate behind him. Did not report incident until emergency room called mine for insurance details.

Employee was scraping roadway with a backhoe and did not see a washed out area, on the side of road, when the backhoe overturned onto it's side.
Employee was replacing a blown hose on a CAT D-11 Dozer and had to drop the front belly pan in the process. The belly pan was supported by a strap which became fouled, creating slack, and allowing the belly pan to drop 6"" - 8"". The belly pan struck the victim in the head, resulting in a laceration that required suturing.
EE was driving SC from miner to feeder hit head on top.
The employee was carrying a bicycle rack with another employee. The employee stepped in a pothole and fell to ground onto his knees. He felt immediate pain in low back.
EE stood up at desk in order to reach for the scotch tape dispenser and when ee went to sit back down the chair had rolled back and away from the desk. As ee sat down the chair was no longer there and the ee tumbled to the ground injuring the back, neck and left shoulder.
Employee twisted right ankle when stepping out of Contractor Truck. Employee did not seek treatment for two days and was back to work within two (2) days at full duty. Injury was not reported as it was uncertain at the time that this was a MSHA related claim; after speaking to MSHA it was determined that the injury should have been reported.
He moved shuttle car anchor up to the corner of the block, he twisted around to pick up the anchor and felt something pull in his back.

The man was loading belt rollers onto a cart when he felt pain in his lower abdomen.
Employees were repairing an air line/manifold to a pneumatic blaster. EE opened the valve on the manifold when the valve the pipe nipple broke and the employee received a blast of air/rusty material to the left side of his torso. The employee was approximately 2' away from pipe opening. EE sustained bruising and abrasions to the left side of his torso.
The employee was helping install 6 inch discharge pumping water line when he slipped and fell breaking his left arm.
EE was operating a loader and got out to do some work on the ground. EE got off the loader and was working on the ground when he stepped backward tripped on a rock landing on his back. Resulting in cracked ribs.
The Dissolver Circulation pump at Power & Production was locking up intermittently. (2) employees were sent to trouble shoot the pump. It was determined the pump would need to be opened. The pump was locked/tagged, isolated & bled. During the removal process when the pump was pried loose from the housing, hot brine was released under pressure that resulted in burns to both EEs.
Employee was lifting and handling supplies and strained/sprained his back. Employee will be off work on doctors orders for an uncertain period of time.
EE didn't start missing time until 9-6-2011 and is scheduled for pre-op appointment on 9-12-2011 for surgery

EE was attempting to remove forks from the battery end of a 636 scoop which was to be sent out of mine. Bolts securing the forks had been removed. EE placed his hands under the forks and attempted to move the forks when they broke loose, pinching his right third and forth finger between mine bottom and the fork, resulting in a laceration requiring sutures to close.

He was driving #86 jeep past #22 jeep that was switched out. When driving by a scraper pipe on #86 jeep caught on #22 and struck him in the face causing lacerations.
IE stepped out of van on 9/4/11 and felt pain in his knee. Was taken to Dr. and released to full duty. He continued to experience pain and saw an orthopedic specialist. An MRI was ordered on 9/26/11 and the employee was diagnosed with a torn ACL on 9/28/11. He decided to seek a second opinion, delaying treatment, and had surgery on 12/10/11 making this a lost time.
An employee was dumping at a stockpile when he backed over a berm. The truck slid backwards and turned over, landing on the passenger side at the base of the stockpile. The employee was taken for medical examination, where he was released to work. On 9/6/11, he had a follow up appointment where he was excused from work.
Employee was opening a can of welding rod when the sharp edge of the pop top of the can cut his right palm at the base of the thumb. Stitches were required and the employee was placed on sedentary duty. Referred to an orthopedist.
Employee was roof bolting when a piece of draw rock fell and hit his right thumb when he was putting the drill still in the pot it fracture his right thumb.
Employee was moving a crusher segment into place. A weld on the segment broke, causing the segment to move and in struck the employee in the right leg.
While backing up the haulage system the wash down hose on the MBC employee was operating fell off the machine and was ran over by the Cat causing the hose to rupture striking him in the face with water spray. He jumped from the operator's compartment and was pinned to the coal rib by the MBC.
Employee stepped over a pipe and felt discomfort in right knee. Incident was first denied as not being work related, comp carrier accepted claim and company was notified on 10/10/2011. Claim is still being contested by company.
EE was pressing a bushing with a piece of steel and the piece of steel he was pushing broke in half and flew into the EE's body, piercing the skin and creating a contusion.
Employee was struck by cable which was being pulled. The cable landed on his foot causing the employee to pull his leg out, twisting his left knee.
Reported turning of steering wheel causing severe pain when trying to use arm to pick up objects or grip objects firmly. EE reported the injury to us as of 12/14/11 for the first time.

Employee was in the face of the #5 entry on the #1 unit assisting the roof bolter operator when his right foot was run over by the back tire as the machine was being trammed up to install another row of roof bolts.
EE was getting ready to get on the elevator to start his shift. There was a speed reducer sitting on the outside of the elevator on the surface that needed transported underground. EE along with another EE picked up the speed reducer to load it on the elevator when EE felt pain in his lower back. EE finished his shift, went to the Dr, diagnosed EE with a pulled muscle.
Employee was loosening bolts on a reducer with a slug wrench and 16 pound hammer. Employee was holding slug wrench when coworker hammered with hammer and hit the employee's right pinky finger. Finger was lacerated and bruised. This information was originally entered on 9/6/11 - forgot to hit submit.
Loading a Pre split shot noticed a burning feeling in groin area. (On 9/19 went to general surgeon to discuss possible surgery options, will update 7000-1 when Mr. Jenkins has his recommended surgery.)
Employee was helping miner operator lift miner cable when he felt pain in his low back.
Injured employee was accidentally shocked by another employee on the neck. An employee held a shooting battery next to the injured employee's ear and activated the detonator next to the injured employee's head causing a shock to the neck. Injured employee was transported to the ER by EMS.
EE was repairing ""dog bone"" on the face side of the pan line when multiple rocks fell between the shields striking the EE in the right leg. The EE received 6 sutures in the right leg. On 9/19/11 injured began losing time due to infection of the laceration. He had not missed any time before this.
On a delivery, EE pulled up to a suspicious vehicle, got out of the vehicle, approached the two men regarding their suspicious behavior, got phone out of the truck, while on the phone writing the plate numbers down, the two men jumped in the vehicle & put the vehicle in reverse striking EE, he fell to the ground hurting his back, the two men drove off.
EE was using one scoop to push another scoop out of a dip when the bucket of the scoop fell off the other scoop resulting in his head hitting the canopy. He continued to work until 9-12-11. Accident was told by injured to not have happened on the job. A medical review was conducted and determined on 11-2-11 to be work related. Safety Dept was notified on 11-8-11 of the review.
Employee had turned off a gasoline fired water pump and was refueling it when the gasoline ignited, burning his left arm, left leg, left side and the left side of his neck. He was flown to Augusta Georga where he is receiving treatment for burns to his left arm (the most serious of the injuries).
The employee picked up a belt roller and when he turned to install it he fell to the ground and was having pain in his lower back and left hip. The employee was not taken off of work by a doctor until 10/10/2011.
Employee was lifting stone out of tub to load saw when he felt pain in his lower middle back. Restricted duty assigned.
Mechanic was climbing onto a Cat D11 Dozer when his foot slipped off of the foot step and he fell landing on his right foot slanted inward.
EE works at Greens Creek Mine and was helping our mill maintenance replace the shafts and paddles in our Nikko Mixer when he was moving a paddle and shaft into position when he felt a pain in his lower back and left leg.
EE was lubricating a tail pulley and reported slipping in mud created by recent rain storms and twisting his left knee. EE reported the incident to his supervisor the following day 9/29/11 as a first aid only and did not request medical attention until 10/05/11. EE was diagnosed with a ""strain"" to the left knee and released to work with restrictions.
He was moving platform, coworker was pulling and he was pushing , he stopped pushing and coworker kept pulling, the platform stopped and his shoulder was jerked away from body, initially a sharp pain to right shoulder and down right arm. This incident became recordable on 10/17/11 when regular restrictions started.
EMPLOYEE CLIMBED UP ON LOADER TO REPLACE WIPER ASSEMBLY. EMPLOYEE HAD WINDSHIELD WIPER ASSEMBLY IN HIS HAND. EMPLOYEE STEPPED OVER ON FRONT FENDER AND FOOT SLIPPED OFF FENDER STEP AND EMPLOYEE FELL AND FRACTURED HIS RIGHT HEEL.
EE had stepped into a two foot trench to help a fellow employee lift a rammer over some underground utilities and when he stepped out of the trench, his knee gave out on him, causing him to fall back into the trench. His knee is the only injury that he received.
Employee was operating roof bolter, he has just gotten into operators deck and turn machine on, he had to adjust his body position in deck and had hold of canopy. The roof bolt machine move and he caught his three fingers between top of canopy and the mine roof.
While walking the associate stepped in the area where Conveyor catwalk and the stairway to Conveyor and the Crusher Platform meet. This is an area with a gap where two pieces of Grip Strut are side by side with one at a downward angle and the upward stairway. EE said that he apparently had stepped the area and heard his right knee pop.
Employee was changing parts from one engine to another when he took a bolt out of an engine mount causing the engine to roll over on its side. He put his left hand out trying to catch the engine. When it fell it caused a deep laceration on his left hand thumb and fractured the tip of his thumb. 7 stitches.
Had lost a finger on screen deck, was lifting cover belting when right went in area where finger was missing. Wedged leg between housing.
The man was operating a scoop along 2nd South Mains. A piece of rock fell from the rib, and struck the man on the right shoulder and neck.
Employee was helping move some iron, and he backed up, catching his left heel on a pallet, falling backwards and straining his lower back.
Injured was helping another EE pull a wood pallet out from under a large bag of rock and felt a pain in Rt. shoulder. He continued to work until 12/20/2011.
Installing a timber on the slope, slipped and caught his balance, but said that he felt a pull in his abdomen at his navel, hernia. This incident was not reported until 10/18/2011 by phone call from witness.
Scoopman was tramming around a corner bucket first, where scoop was at it was high and where employee was at it was low so scoop driver did not see employee kneeling down cutting tie wire and employee also had ear plugs in and didn't hear the scoop and the bucket came down and hit the employee in left leg.
Employee went to the mezzanine above the jog line (bagging line 9 - packaging department)to perform a change over in products on deck 1. He began to walk across the other side of the deck and slipped. As a consequence, he fell and hit his head on the side of the metal wall. Employee has a head injury; a cut to the left side of his head.
Employee states that he was lifting a lube pump out of a bulk lube tank when he felt pain in his neck and shoulder.
Employee was descending ladder from a haul truck and stepped off ladder on to a baseball sized piece of shot rock on the ground with his right foot. This caused his right knee to pop and immediately he felt pain. The haul truck was located West of the fuel pump station.
Employee was taking a 5 gallon bottle of water up the stairs to the fifth floor of the tower. He felt a pop in his right knee and was in alot of pain. He was taken to the doctor and was diagnosed with internal derangement of the right knee.
Employee was performing normal duties of a weighmaster which included tearing ticket tags for customers, when she developed swelling and pain in both wrists. When reported to the company, the employee was given modified duties that were accommodated until 2/17/12. Her condition did not improve so modified work was ceased to allow for additional healing.
Employee said he was attempting to roll a rock off to the side of the haul road.
Employee was loading parts, felt pain in r shoulder. He did not seek medical attention at that time. Visited doctor on 1/31/12 and was prescribed physical therapy making it reportable as of that date.
Injured employee was hit by a hammer from a distance of 4 feet. The hammer came out of the rubber protective sleeve on the handle and the metal hammer flew through the air and hit the employee on the left hand. Resulted in a contusion and a broken bone.
Employee stated that he was digging rock off of the coal seam. The loader struck some tight rock and came to a sudden halt causing a whiplash type motion to the employee's neck area.
Employee threw a 32# piece of shovel wear plate into the back of a pick up truck and felt soreness in his shoulder.

Employee slipped in mud while walking to his work station on 9/15/2011. The fall exacerbated a chronic back condition of low back pain. The employee reported to his supervisor on 11/07/2011 that his back condition has not improved and is now missing work secondary to this back pain.
Right little finger was pinched between a chain and backhoe bucket while trying to release the come along from the chain that was securing a concrete block in the bucket. The block tipped forward tightening the chain and pinching the finger.
EMPLOYEE WAS AT THE REAR SECTION OF AN 8"" PIPE THAT WAS BEING PULLED FROM A TRENCH BY A 824 JOHN DEERE LOADER. THE REAR OF THE PIPE SUDDENLY CAME UP OUT OF THE TRENCH AND STRUCK HIM IN THE CHEST.
While employee was running to the restroom, he tripped/slipped on a rock and twisted left ankle causing a sprain to left ankle.
Hoistman was descending the stairs from the hoisting platform when he slipped and twisted his left knee, aggravating a pre-existing condition (torn meniscus). A MRI was performed on 10/14/11 and employer was notified of condition 12/1/11 making this reportable.
Miner (mechanic) received a metal sliver in the little finger of his left hand - did not notice until hand began to swell - unsure how he got sliver
Employee was getting off the dozer when foot touched the ground, his (L) ankle rolled.
Employee claimed he was repairing a hydraulic leak on Cat 62 heard a pop in his knee and awoke the next day with a swollen knee. Time cards indicate he worked a full 12 hour shift the day after he worked on C62. He never reported any incident or injury until he failed to report for work 2 days later

Torn bicep muscle on right arm from lifting a piece of piping
On 9/11/11, employee was rolling a 2 yd mucker tire in shop, when tire hit divot in ground & it made the tire spin & fall into his Left knee. He went to medic for evaluation. all was ok. no days were missed. In November, employee went to doctor cause knee was still sore, surgery was set for 11/18/11, 1st shift missed was on 11/22/11.
Employee fell and hurt L/shoulder on accident date. Continued to work regular job until 10/15/2011 Lost time started 10/17/12011
EE was adjusting the loading bucket cones at alumina loading dock. He was doing this by pulling them up and repositioning them. EE felt a strain in his left elbow. EE reported to the mine medical facility for evaluation. EE placed on restricted duty on 9/23/11. EE lost work days beginning 3/6/2012 to 4/13/2012. EE RTW full duty on 4/13/2012.
The miner was working in the preheat tower cleaning and water blasting. He reported that his shoulder and arm were aching. The employee was moved to operating a loader for the remainder of his shift and the next night. After that he continued with no problems. He has since complained about his shoulder hurting again and he is now on restricted duty.
Walking and stepped in a hole that had a wedge in it, tripped and fractured 5th metatarsal on his right foot.
The incident took place in the pit at the dragline. Employees were upending drag cable. employee was setting socket by pulling cable through the beckets. While he was pulling he felt a pop in his right arm. Employee suffered torn right bicep.
Employee was handling 50 lbs. bags of dust; while tossing those bags he strain/sprained his shoulder. Employee was taken to the doctor and was ordered from the doctor to remain off work. Employee needs surgery and will remain off work on Workman's Comp for an uncertain period of time.
Rock fell from between the shields, striking employee on his left hand. Reportable lost time injury as of 09/09/11 for surgery.

EE WAS LOADING EQUIPMENT PARTS ON A FLATBED TRAILER WHEN HIS FOOT SLIPPED OFF THE EDGE OF THE TRAILER DECK CAUSING HIM TO FALL AND BRUISE HIS RIBS.
Production was locking up intermittently. (2) employees were sent to trouble shoot the pump. It was determined the pump would need to be opened. The pump was locked/tagged, isolated & bled. During the removal process when the pump was pried loose from the housing, hot brine was released under pressure that resulted in burns to both EEs.
Carrying steel grate with second employee, who dropped grate. Injured employee then dropped his side of grate, causing grate to come into contact with his left foot. Brusing and swelling of left foot at this time. Further evaluation necessary off site.

Employee was walking up the beltline when right knee gave out and he felt a sharp pain in right side of knee. Worked regular duty until he had knee surgery on 1-18-12, becoming a lost-time accident.
