Accident type, without injuries

ROOF FALL - Occurred on the No. 1 longwall. The fall was approx. 200 feet outby the longwall face in the No. 1 entry and fell above the roof bolt anchorage. The rock fall was approx. 20 feet long and 10 feet wide and did not block travel or impede ventilation.

Contractor with Helsman Sweeney Company was entrapped in the 1141 passenger elevator due to elevator door inadvertently stuck in closed position. Contractor exited elevator at 11:20 AM.

An inundation of water occurred after a pilot bore hole was mined thru. the inundation affected the #1 Section, specifically entries #3, #11, and #12.

A roof fall was discovered at approximately 12:45 PM on January 30, 2015. The fall was located in the 1st Southeast Main, xc 41, between No.3 and 4 entries. It measured approximately 55' x 18' x 8'. The fall itself was not reportable but because the adjacent roadway was closed and the road rerouted it was reported to Federal and State agencies.

At approximate 6:20 pm, an unintentional roof fall occurred on mmu 005-0 in No. 2 Entry. A piece of sandstone broke into 2 pieces and fell in front of and blocked MRS. No injuries, ventilation not disrupted, escapeways not blocked. A roof control plan to prevent re-occurrence was submitted and approved by MSHA and MHST.

AMS operator discovered that Deer Creek hoist had lost operational power. Due to surface travel distance to the site, the hoist was inoperable for more than 30 minutes. When personnel arrived at the site, they found a circuit breaker had tripped. The circuit breaker was reset, restoring power to the hoist.

The elevator at the #6 portal went down at 7:05AM. It was 80' from the bottom. There was no one trapped. After inspecting the unit, it was found to be out of calibration. The unit was re-calibrated and back in service at 8:45AM.

Transformer fire started at the Avista electrical substation in Osburn, ID all power out at the Galena mine. 49 miners were notified at 0130 hrs. to move to level stations. Coeur Hoist was manned by a hoistman. Power was diverted by Avista to the Galena #3 Hoist at 0355 hrs. All miners on surface by 0510 hrs. Day shift has been cancelled partial power in place by Avista.

A roof/highwall fall occurred in the number 4 intake fan entry between the end of the fan house and highwall. Fall/slip was approximately 14' wide x 3' thick x 5' high. Fall was not above anchorage.

The PLC on the Silver Shaft Hoist lockup up. Gave the hoist man a warning on the computer screen and the hoist would not run.

Pleasant Grove Elevator was down intermittently from 6:30 AM to 8:00 AM due to the door brackets and rollers needing adjusted.

The elevator at Emerald #1 mine at #1 portal went down at 3:50 am on January 27, 2015 and was repaired and running by 6:15 am that same shift. The midnight electrician found a stuck buffer switch and repaired it.

An unplanned sulfur fire occurred on the 4450 level of the C4 south wall at 6:30 am. No equipment, no structures and no miners were put in harm. There was an existing sprinkler system in the area cooling hot spots. MSHA issued a 103(j) order at 7:40 am to prevent destruction of evidence. Sprinkler system extinguished fire at 8:25 am.

Unplanned roof fall - seal set #45 1st west main involving intersection of the #4 seal/ No ventilation was affected. The are of the fall has been dangered off. Supplemental support has been added to the area to assure safe walk way to the #4 seal. The #4 seal has been examined and no conditions are present that would compromise the integrity of the seal.

A non-injury incident occurred when the automated hoist car mechanism malfunctioned at 12:15am. Electrician was immediately dispatched. Both section crews were withdrawn to point outby the loading point until repairs were completed. Electrician arrived at 12:20am and repaired hoist car for manual operation by 12:30am (15 mim). Complete automated repairs were completed at 12:53am.

A fall of the roof occurred on the 3rd N.W. Panel, crosscut 30, #5 entry, intersection, LOC. Fall measured approximately 15' width by 20' length by 8'-12' height.

At approximately 7:40 a.m. the elevator was removed from service for more than 30 minutes due to a mechanical issue.

Employee was out on deck of a D11T Dozer when he noticed smoke coming out of the front of the machine. He turned to get fire ext. and when he turned back the fire was coming out of engine compartment. Employee shut it off and got off the machine.

An unplanned sulfur fire occurred on the 4550 level of the C4 south wall at 7:20 pm. No equipment, no structures and no miners were put in harm. An access ramp was pushed in with dozer and sprinkler connection was completed.

Shaft crew was lowering VFD unit under conveyance in service shaft. Sling eye failed shifting weight and causing VFD to bind in shaft and fall aprox 250 feet to bottom. No injury or entrapment. Damage to VFD and 2 guide sections. MSHA hotline report within 15-minutes

Unplanned Roof fall at 5 MN xc25. Fall occurred in intersection (4-way)slip plane that was supported with 72"" TR bolts and 11 ft. cable bolts. fell from roof. Fall dimensions 19'W x 30'L x 8'-10'H. Fall did not cause injury, impair ventilation, or entrap miners.

The production hoist tripped to manual on a converter fault. The control board on the east converter was found to be bad. The board was replaced, power was restored, and test runs were performed.

At 7:05am the elevator got stuck on the bottom of #9 portal. The electrician was notified and it was found to be a blown fuse. There was no one on the elevator at the time of the incident and no one was trapped. the elevator was fixed and running at 8:10am

An examination of the emergency escape facility was being conducted and after the upper level over travel test, the hoist malfunctioned and would not operate. During the troubleshooting process it was discovered that a 24 VDC relay had a bad set of contacts and the top landing switch was not making contact properly.

Had a roof fall in the old works on 2nd West Mains in the # 1 Entry at spad # 71542, the fall was approximately 8 ft. high 25 ft. wide, and 20 ft. in length. The area was dangered off and used timbers, prop setters for additional support, and moved stopping line over to make travelway.

361 experienced sudden failure on truck. Operator parked & engaged the suppression system, exited cab with hand held. Rear engine was on fire approx.60'from operators cab, moved to a safe area. No water used in-order to control leaks. Failure: drive-train area. The shaft & yoke come apart and destroyed guards, battery cables and hydraulic lines, ignition caused by battery cable.

A roof fall (75' long X 6' High) was found on the weekly examination in the 6-North Main left return between spad # 7842 & spad # 7513 in # 2 entry. This did not involve any compliance issues, no equipment was involved and no other 50.20 criteria. applies. The cause of the roof fall is undetermined at this time. Fall will bolted and walkway restored.

A continuous miner operator was loading coal in entry #3 on #1 MMU. As he finished his second lift a methane bleeder was ignited. The flame was extinguished without further incident. No miner was injured due to this methane bleeder.

The guide rope at the escape hoist located at cross cut 89 on the west mains was found damaged. The guide rope was replaced and the escape was put back in service on 01/20/2015 at 1:15 pm.

Examiner found the fall in the belt entry (#4)@ # 52 cross cut on the early exam for 3rd shift. Initially the fall was approx. 30' by 30' by 12' high. It appeared that the slickened sided slips along with some verticals lined up in intersection/crosscut.

Lost the end coders on the hoist. Electricians arrived and reset end coders. Hoist back up and running at 8:00 A.M.

A roof fall was found in our 5W-D Mains at crosscut 22 in the secondary escapeway. The fall did not cause injury to miners, impede passage of miners or affect ventilation.

A laborer returned to #7 Burning Floor to rake grates. Moments after inserting the rake, EE heard a loud sound and felt a gust of air from the kiln. Upon investigation, it was discovered that this sound was the `rupture disc rupturing in the duct work from the #7 cyclone that was caused by a surge of air through the kiln. There were no injuries associated with this event.

A roof fall was found inby the section loading point on our #3 MMU, entry 1to2. The fall did not cause injury to miners, impede passage of miners or affect ventilation.

A piece of ice fell down the intake side of shaft cutting a remote line to the escape hoist. All men were evacuated from mine until repairs could be made. Upon completion of repairs and test were done on hoist men were allowed to return to work

Tilden PP Elevator #2 stuck on the fan floor. 11 persons were inside the elevator for about 1 hour. The elevator repair service and MSHA were called almost immediately. The elevator repair service opened the door at about 4:30 AM. MSHA was contacted and notified that the persons had been released.

The elevator was in route from the mine bottom to the top when it stopped. The track of the door had some dirt in it so it was cleaned and the access roof door was found to be ajar and it was closed and the elevator worked following these findings.

A roof fall occurred in the #3 entry approx 10ft inby the mine opening. The fall area was cleaned and re-supported. MSHA examined the area on 1/20/2015. No injury occurred. Escapeways and ventilation were not disturbed. Fall was approx 10'x15'x3' & 3 bolts dislodged.

Pleasant Grove Elevator was down due to a bad wheel. Vibration was felt and elevator was inspected until it was determined a wheel needed replaced.

Hoist contactor bumper pad came loose. Miners were evacuated at 10:45 pm. Evacuation completed at 11:00 pm. The hoist contact pad was repaired and miners returned underground 12:55 am.

Found an unintentional roof fall in the #1 entry intersection of Calvary 80 Mains and Leslie Perry Mains. Roof fall dimensions are approximately 20' wide by 40' long by 5' thick.

Something dislodged in Aux Hoist, falling and cutting control wires at bottom. Inspected and found that wires were cut at bottom and that it would take more than 30 minutes to fix. Evacuated mine and then made corrections to cage.

The area in which the fall occurred was in the 5RT. panel. The continuous miner had finished mining in the No. 7 entry when the roof fall occurred.

During an inspection of the production hoist the employees tested the upper limit switch and the hoist tripped out but was unable to be reset. The employees remained on the inspection deck during troubleshooting of the hoist but were able to exit the work deck if needed via a ladder to the next level of the head frame. The brake switch was reset and the hoist became operational.

The door switch on the 5th floor of the passenger elevator failed. Two employees where in the elevator at the time and the car stopped 18""above the 4th floor landing. The outer doors would not open, MSHA was notified at 1:00am of the situation and Murphy Elevator Company was called. The employees where removed from the elevator at 2:30am. No injuries resulted from this event.

An unplanned roof fall occurred at approximately 6:00 PM. The fall was at cross-cut # 32 on 6 South track. The fall was about 10 ft. high, 80 ft. long, and 20 ft. wide. The fall was timbered and dangered off and it will be cleaned up.

Fall started with an unbolted face falling in and then propagating into the intersection in the #2 entry @ #52 cross cut of the West Mains. It appears that slickened side slips were present and lined up in the intersection. Fall was approx. 40' long and 30' wide through the intersection and approx. 12' high. No personal injury associated with the fall.

IGNITION - Occurred while cutting mine floor in the #4 entry on 6 section E10 panel. They were cutting mine floor and approx. 10 feet in bottom outby the face on the off curtain side of the entry. The flame lasted approx. 10 seconds and was put out with one fire extinguisher.

The Emerald #1 Elevator went down when ice which had accumulated in the shaft broke loose and damaged two buffer switches. Schindler Elevator arrived at 1:45 PM and replaced the buffer switches and the elevator was back in service at 3:15 PM.

The Otis elevator at the coal silos tripped out with an employee stuck in it. Another employee reset the breaker an the elevator continued down. It was determined that the gear box oil was thick due to the low temperatures that night. No mechanical issues were found.

The Cameron elevator when down with 2 persons entrapped for 39 minutes. Grease on the elevator rope caused the governor to trip. Cleaned rope.

A roof fall was discovered in the #1 panel off the Big Bear sub-mains during a weekly examination. The fall was apx. 90' long X 20' wide X 4' to unknown height. The fall was dangered off and reported to MSHA. The roof fall will not be cleaned up and a ventilation supplement has been submitted and approved.

I believe a slip feature located above the roof bolt anchorage and not visible to the miners caused the immediate shale roof to fail. Roof fall is about 50' long, 18 1/2' wide and 5' to 8' in height. No men or equipment was involved. Ventilation was not interrupted.

A roof fall was discovered in the track entry at crosscut 44 on #11 mains during a pre-shift examination. The roof fall is apx. 60 ft long X 7'high X 18' wide. The area was dangered off and an action plan submitted to MSHA. The fall did not affect persons, ventilation nor equipment. The fall will be cleaned up.

A fire was reported to MSHA 01-05-15 at approximately 10:40 pm in the pit. MSHA issued a 103(j)order to prevent the destruction of evidence. Upon MSHA inspection at approximately 1:00 am there was a small hotspot on the south highwall.

An unplanned roof fall was discovered on 1/5/2015. The fall was located in the South West Mains 1 in the #6 entry; #17 to 18 block; inby spad #995. The fall measured 18'W X 75'L X 6'H. Weak laminated shale roof that was layered with sand streaks and deteriorated over time.

Employee was stuck in elevator starting at 3:10am. Supervisor was called at 3:25am, guard shack at 3:43 am. Employee exited at 4:08 am. Guards cleared scne at 4:16am. Safety was notified at 4:16 am, MSHA called at 4:24 am. Employee experience 38 years/32 weeks. 39 employees on site/0 contractor.

While mining in the #8 entry of the 7 North Right section an ignition occurred while sumping the continuous miner head just below the binder. Two bits were found damaged on the return side of the head in the inner ring of the drum extensions. No persons or equipment were injured/damaged as a result of the ignition.

Roof fall in the #3 entry, crosscut #13, 13th Right Section. Roof fall was in the intersection, dimensions were approximately 42 x 27 x 17 feet high. MSHA Hotline was called.

Repair back brake adjuster. The hoist car was reported down at 9:30am. The back brake adjuster broke. There was no one trapped and the mine mechanics, as well as the contractor, repaired the hoist. The hoist was back in service at 8:00pm

Unplanned roof fall discovered in 21D 1A entry at 2 block intersection. Fall approximately 50' long, 20' wide and unknown height. The fall will not be cleaned up, has been dangered off and the perimeter supported. No problems with mining methods, machines, training, ppe, or compliance issues.

Miner Operator was tramming into cut. When he positioned the miner onto sights, the mining machine was on a knuckle on the bottom causing it to swing in his direction, pinning him against the rib with the tail of the mining machine.

Drowning

The victim was operating an excavator near a canal when the excavator tipped on its side and went in the water. The victim was able to get out of the cab and transported to a hospital where he died at 5:45 p.m.

Struck by falling object

EE was fatally injured while performing the standard practice of scaling loose material using a mechanical scaler. Loose material fell from the back contacting the scaler and employee.

Struck against stationary object

The employee was fatally injured when he was travelling on a shaft conveyance and came in contact with the shaft furnishings.

Caught in, under or between a moving and a stationary object

Knife River miners were replacing a screen. Our sales representative was on site. He was on site to provide technical assistance. He was struck in the head by the feeder box. EMS was notified and responded. A Knife River operator notified MSHA within 15 minutes. Injured person was taken to the hospital where he then passed away.

Caught in, under or between running or meshing objects

While machining parts the employee reached with his gloved hand to swipe away metal chips when the cutting head pulled his hand into the mill severing his right middle finger.

Employee was installing roof bolt when the bolt didn't align with chuck wrench. He then had to slow feed it in the hole. Once he started feeding the rotation was too fast and it caught his glove and tore his thumb off.

Struck by falling object

Employee arrived at the section to restock the CO#247 roof bolter. As he was walking away from the bolter after checking what supplies were needed, a piece of draw rock fell from the roof striking employee on his right foot. The resulting accident required the complete amputation of all 5 toes on the right foot due to their damage from the falling draw rock.

Over-exertion in lifting objects

Loader Operator lifting new scraper blade for installation on loader bucket. Result was small umbilical hernia. The employee has continued work with no time lost to date. Workers Comp has referred employee to physician to determine type of treatment that may be necessary. Unknown at this point if any surgery is required.

Struck by falling object

Worker was picking up a chopped rock with a sharp edge to put on a pallet. It slipped out of his hand and landed on the end of his ring finger on the left hand severing the tip of finger off.

Over-exertion (Not Elsewhere Classified)

Employee was performing maintenance on sand screw when employee slipped; while attempting to catch himself put his hand between the sand screw and a skid steer rake causing his finger to be lacerated and crushed, and subsequently partially amputated.

Employee was attempting to marry a fan silencer to a fan. In the process he pinched his right index finger between the silencer flange and cradle.

While changing a seat base in truck, employee needed to unhook the wiring harness which had a cable tie on it. As Employee cut the cable tie the knife slipped resulting in a laceration to his right index finger. Employee was treated at CAMC General Division where the right index finger was amputated below the finger nail.

Operator tried to touch drive belt, machine was off but slowing down, and had 3 fingers smashed by belt pulley, severing tip of middle finger. Operator put his hand on the existing guard.

Employee stated that he sprained his right ankle when he stepped off of the rock dust motor. He started his shift on 1/30/15 at 11:00 PM.

Employee, was exiting the Kenworth, tri axle, dump truck at the end of his shift. He slipped on the truck steps and twisted his right knee when his leg hit the ground.

Employee stated that he was working in the shop putting a seat in a coal loader. He squatted down to put in bolts and when he tried to get up, he felt severe pain in his left knee. Checked by doctor on 02/11/2015 due to persistent pain and only became reportable on 02/20/2015 when he missed his first shift of work.

Over-exertion in lifting objects

While installing mainline structure felt a strain in his lower back while putting stands on ropes. Came to work on 02/02/2015 and worked every day until 03/30/2015 then went off

Struck by falling object

Drill wrench was bent causing the drill steel to impact the employee's right palm/wrist.

Miner was performing routine tasks carrying samples in a 5 gallon bucket down some steps and felt a pull in his shoulder. Minor first aid was performed and the miner claimed he felt fine. This injury didn't affect his job until 4/17/15.

Area was very muddy. Section was about to start moving to set of rooms outby. Haulage system was being disassembled. Bridge conveyor was removed from RHS and dolly and set on a crib block on the bottom. Mechanic stepped between bridge and RHS when bridge slid toward RHS catching victim's left ankle and fracturing same.

Slipped on Ice while exiting the haul truck. Bruised hip

Employee was injured when he slipped and fell into the operating longwall face chain.

Scale clerk was waiting on a truck at the ticket window, she turned to go see the location of truck at which time she fell. Clerk indicated as she turned her ankle gave way and she fell in the middle of the office floor. As a result of the fall she has 3 fractured bones in her left foot.

Absorption of radiations, caustics, toxic and noxious substances

Injured was plastering a stopping when plaster splashed back and got onto forehead. The plaster then rolled down the injured face into left eye.

While opening an airlock door, the employee lost his balance and fell to the ground. The accident occurred because the employee was not paying attention to the task he was performing. The employee did not violate any rules, regulations or policies. He had on his appropriate PPE materials. He has a middle of the road attitude.

Was snowing #6 head roll spinning out. Climbed onto unprotected elevated spot to heat head roll using a weed burner. The tip of the weed burner came into contact with head roll pulling employee into head roll catching sleeve between head roll and conveyor belt causing right shoulder to come in contact with receiving box shroud.

While the operator was checking his tractor for a leak in a hose, his left index finger was pinched between the hose and the frame of the tractor. This operator has been hospitalized for an infection in the left index finger.

Employee was getting off a track hoe he was running, and stepped upon uneven ground. He rolled his right ankle, fell onto right knee, and fell on right shoulder.

Employee was walking through supply yard and slipped on patch of ice hyper extending right knee. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

Over-exertion in pulling or pushing objects

While trying to shove a hose in drain line on spirals that was stopped up he twisted his back. Employee did not miss a scheduled work until 2/9/2015.

Struck by falling object

Dredge mining winter repair - replacing and realign #3 sand screw liner plate - weight 300-400 lbs. was 4' X 8"" X 1/4"" steel. Dropping liner plate into screw box landing on hand of employee.

Removing a vertical pin on a relay bar at 127 shield with a sledge hammer. He was laying on his side, swinging the hammer with his left hand (right handed) and missed, striking his gloved right hand, resulting in contusions. No problem with PPE, training, mining machines or compliance issues.

He was given eye drops as a prescription and was told to follow up with the doctor on 1/28/15. I got the e-mail on 6/29/15. The first day of vacation.

Employee was helping to carry a conveyor belt bed rail with another employee when he tripped and fell striking his left leg just below the knee on a metal rib strap that was on the mine floor. Caused a laceration that required 7 sutures to repair.

The accident was caused by the failure of the employee to observe, recognize the bottom conditions and take the appropriate preventative measures. INJURED WAS TRAMMING A SCOOP, HIT A HOLE, AND GRABBED TO CATCH HIMSELF FROM HITTING THE BACK OF THE SEAT AFTER THE BOUNCE. HIS LEFT HAND WENT BETWEEN THE DECK AND REAR SECTION OF THE SCOOP WHEN IT WAS CLOSING TOGETHER. THE TIPS OF HIS RING FINGER AND MIDDLE FINGER ON HIS LEFT HAND GOT CAUGHT BETWEEN THE TWO SECTIONS. STITCHES WERE USED TO CLOSE THE ENDS OF HIS FINGER TIPS.

EE was putting dirty filter sectors from a pallet into the sector washer. When doing this, the operator felt a sharp pain in her back. EE was brought to the hospital for an evaluation and at which time she was taken off of work for one shift. She returned to full duty 1-12-15

On 01/13/2015 the employee sought medical evaluation where he diagnosed with a neck strain and was given a restricted duty release to not wear his fall protection harness.

The EE was mining in #5 heading. He pulled the cable away from the miner and walked towards the rear of the miner. Through the investigation, it was determined that he walked past the standoff while the pump motor was still energized & accidentally pressed the tram lever resulting in the miner pinning his midsection against the rib.

Rock fell out from between straps striking EE on head and shoulder. EE was taken off work on 1/8/2015 due to spasms and limited motion.

Driver wedged the shovel between gates but gates on his legs caused a broken right leg and soft tissue damage in the left. Note: the reporting period for the 7000-1 was inadvertently overlooked.

EE removed approximately 3 boxes of 40 lb finished product from a conveyor. A short time later EE reported to his supervisor that his R shoulder was sore. Incident initially classified as near miss. EE sought diagnostic treatment in May which determined a rotator cuff tear. EE went out for surgery on 6/5/15.

One of the employee's associated with the other organization utilized a hammer to contact the roller and that is when a sliver of metal entered into our employees' upper torso area.

Two pumpers were connecting 6"" pipe. The track hoe was connected to the opposite end of the connection. After communicating his intentions, the other pumper began to align the gasket. As he aligned the gasket, the track hoe operator was startled by material movement behind him. This made him hit the machine's swing, causing the two pipes to touch, pinching the employee's wrist.

An employee at the plant was removing a drive belt from a motor for the feeder pan pulley and pinched right ring finger between the pulley and drive belt. Causing fracture to right ring finger.

Employee was operating a scoop at the end of the track scooping up a motor to load on a flatcar. He backed the scoop in a break, when he did he backed over a timber 6x6x8 ft long. His head hit the side post of the canopy on the scoop.

EE and another ee were removing bolts from a pump on the prep plant. EE was using a pull bar to break the bolts loose. As he strained, he felt a pain and burning to his lower abdomen and groin area. No visible injury: however he has had 2 prior hernia surgeries. He was taken to Urgent Care.

Employee was lifting feeder cable to be hung after feeder was advanced in preparation for a belt move. Cause of accident was a small cut in cable and employee had damp gloves on. Surgery was performed on 1/6/15 and a no weight restriction was placed on associate for eight weeks.

The individual closed the vehicle door and began walking toward the walkway near the jersey barriers when she slipped on black ice and fell down on her side. The individual injured her ankle. She suffered a minor fracture to her left ankle.

On 01/5/15 an employee slipped on an HDPE liner while fueling a work truck. The employee fell and landed on his left arm. An MRI revealed that the employee tore one of the muscles in his left rotor cuff. Surgery was required on 3/10/2015 to repair the damage to his shoulder. The employee is a supervisor and was able to work at full duty prior to the time of his surgery.

Over-exertion in wielding or throwing objects

Emp was changing silo 23 to silo 20 and was hitting selector in an upward position w/slegehammer w/Rt.hand. Emp struck Lt.hand index finger. X-rays showed three fractures and a splint was placed on finger. Restrictions of ""no Lt. arm work"". Emp continues at in his same position training a new PA. No lost time.

While climbing a 4' step ladder, employee strained his left knee. No unusual behavior was noted, just the knee failed

Fall to the walkway or working surface

Employee slipped on ice in upper parking lot after getting out of car. Right ankle sprain.

Bodily reaction, (Not Elsewhere Classified)

EE was in dry at end of shift after shower and changing into street clothes; bending over felt his left knee pop. Was seen that evening, released to duty. Was seen again on January 12 - put on restricted duty awaiting an MRI.
