Center alternative fuel pile caught fire inside AFR shed for reasons unknown. Subsequent piles inside shed also caught fire. Fire depts called, MSHA hotline called. No injuries.

The 20 man elevator was out of service from 11PM on 4/30/17 until 2:30 AM on May 1st. A shorted wire in junction box in sump pit plus an open wire in trailing cable was found and repaired to put car back in service.
We had an inundation of water on TG#2 at xc132. The water came from a borehole that was drilled for a turbine pump but was not installed yet. This was caused by the extensive amount of rain and flooding we received from a 100 year rain event from 4/28/2017-4/29/2017.
Rainfall event far exceeded national standard for Bottom Development Plan standards. Pumps were unable to keep up with the amount of water coming in to the pit.
Borehole with a pvc pipe in it allowed surface water from a field to enter mine at the 2nd East Mains Parallel - entry 11 - approx 17+65 Sta. Water gathered at the end of the 2nd East Main Parallel. It appears that a farmer knocked the top off the pipe. The borehole appears to reach the 6 seam only. Wildcat Hills did not drill this borehole. In response to citation # 9105311
The production hoist tripped out on a heartbeat fault and could not be reset.
The elevator went down at 1:07am, there were 12 people on the elevator when it went down, the issue was the over travel switch and short circuit in the e-stop. There were no injuries that occurred.
On 4-28-17 at approximately 5:19 a.m. an employee was riding the service hoist when it went down due to electrical issues. The hoist then started running again and then went down for a second time due to the same electrical issue. The hoist was repaired by changing a contactor and no one was injured.
State elevator inspector wanted the pit door switch moved to the striking side. Once the switch was moved to the striking side, it took on water causing it to fail. During maintenance the alimak car traveled down the shaft 50ft without anyone in it and tripped a circuit breaker.
EE was taking the passenger elevator on the preheater tower when a gust of wind moved the power cable and got attached to 8"" piece of conduit. The power cable wrapped itself around the conduit and pulled the power cable apart, electrically disabling the elevator car. Fire rescue and Executive Elevator had to be called to rescue the EE from the elevator car.
Found an unintentional roof fall in the #1 entry just inby xcut 21 extending inby between xcuts #23 & #24. Fall is approximately 140' long by 19' wide by 8 to 10' thick.
There was a kink in the hoist rope near the termination point at the hoist car. The kink was removed and rope reentered into the hoist car.
A roof fall occurred on a worked out section not an active section in #1 entry spad #7850. 2 pins wide, 4 to 5ft high, pulled 5ft fully grouted pins, 20ft long, and 12ft in width.
A fire at the Mammoth Magnetite Plant storage belt stacker. The plant was being deconstructed by Powell Construction. At approx 6:20pm a pedestrian traveling Rt 60 noticed smoke and stopped by Mammoth Guard Shack and notified the guard on duty. The time of the fire and duration are unknown, however minimal damage occurred.
Electricians checking Galena Hoist MG set, found one brush on MG set broken off. Back up and running at 7:15 AM April 18 2017
On 4-15-17 the longwall headgate became impassable due the top setting down on the longwall stageloader. By 4-17-17 the wall had ""pushed"" enough of the material away to expose the strata that showed the top had fallen higher than the roof bolt anchorage zone making this a fall by definition.
MMU 058 experienced a roof fall at approximately 51 crosscut on the longwall headgate drive. The fall was approximately 20'L x 16'W x 6'T. The fall will be mined thru as part of the longwall cycle according to the MSHA approved plan. The roof fall reference number is 8.
A water leak has occurred on the #2 section in the #1 entry. The water is leaking from the mine roof and adjacent pillars of the #1 entry. No injury occurred during the water leak. MSHA hotline was utilized and district personnel arrived at the mine site shortly following the report.
At approximately 9:25 am a small fire around 1 foot in diameter was discovered while some repair work on the long wall stage loader was being conducted. The fire was located at the top of the Mary Lee coal seam between the coal seam and the main top.
Air boat 1 launched for survey. Survey began air boat shut down w/4 EEs aboard. Attendant notified; notifications made; plan formed to launch second AB for retrieval. Air boat launched. When they arrived to AB 1, AB 2 quit running. Mayday called; Sup. Instructed 2 EEs to get marsh master w/ paddle boat, plywood & rope to retrieve EEs. Marsh master overheated; EEs were removed.
A fall of the roof occurred in the 1st NE Parallels, #00 entry, crosscut 19, intersection. The fall measured approximately 20' length, by 20' width, by 7'-12' height. The area will be sealed off by the construction of A-Seals.
Cave was found on a pre-shift exam in section. Fall was approx. 90' long and 18' wide and approx. 5' high. The fall did not impede passage or impair ventilation. The immediate roof around the fall appeared to be in good condition. Both Federal and State inspectors investigated the fall on 4/10/17 and 4/13/17.
The E29HG crew experienced a face ignition while mining. It lasted about 30 seconds. There was a clay vein that went across the face and it emitted methane on the left side of the miner. MSHA also citied worn miner bits and low water pressure.
The jack rope gripper faulted due to an oil leak. The elevator hoist was out of service at the surface landing until the jack was filled up with oil. There were no injuries or entrapments as a result of this occurrence.
Production hoist was out of service due to a PLC loss of communication.
Three maintenance mechanics entered the Kiln Feed Silo personnel elevator to descend to the ground level. Shortly after the elevator started to descend, the elevator abruptly stopped, trapping them inside. Thyssen Krupp was contacted. TK placed the elevator in maintenance and brought the car to the closest floor and allowed the team members to exit safely.
Trailing cable on the Koepe hoist became twisted and kinked the cable. All employees were safety evacuated out through the production skip. A new Koepe trailing cable was installed.
Sparta Elevator was down due to a malfunctioning door switch. No one was on the elevator at the time.
A brush fire at the reclaimed hill at 1525. 911 was called at 1530 the fire was contained at 1645. No damage to the property, equipment, or danger to employees or residents. Fire was caused by light reflecting off polished tin covers & hitting the cardboard cover, igniting it. Wind speeds at 15-20 caused the inflamed cardboard to spread to the dry grass igniting it.
On Friday 04/07/2017 two persons were in the preheater tower service elevator when it became stuck on the 5th floor where its doors would not open. This condition lasted approximately 60 minutes until the elevator service company responded and manually released the elevator doors.
Lost power feed from Cleveland Public Power to both Hoists.
Main power loss to the 8 shaft facilities caused power loss to the 8 shaft hoist. One miner was on the hoist when power was lost and was trapped until power was restored. The miner was trapped for a total of 45 minutes.
The counterweight guide shoes were worn allowing the counterweight to separate within the guide rails. This caused the counterweight to strike a cross beam damaging it. It also caused the two (2) connecting pins that connect the top and bottom counterweights to break apart. This was repaired and put back in service that day.
At approximately 10:18 a.m. an employee was grading bottom with a continuous miner in the 5Rt. face and saw a yellow/orange flame around the center drum. At this time everything was shut down. No one was injured and during the investigation at this time there was no clear evidence of an ignition.
At approximately 4:35 pm an issue with the valve feeding pressure to the pinion brake cylinder on the #1 hoist lead to a failure of the brakes to release when the hoist was operated. The issue to the valve was identified and repaired, and the hoist was extensively tested before being brought back into service at approximately 5:46 pm.
While moving manlift from edge of concrete to dirt road, employee was launched into air about 3.5 ft and came down hard in basket hurting left knee. Employee was checked at doctor, no injuries and was prescribed over-the-counter pain medication.
Sending supplies into the mine via the #3 cage w/ 5 ton overhead hoist. The material sled was halfway out of the cage at the landing when the springs around the chain/hook assembly became entangled in the hook causing the chain to not properly pass through the hook. The sled could not be freed initially causing the #3 hoist to be inoperable.
A phase was lost on the power pole that supplies power to the production hoist. The loss of phase caused the production hoist cool fan motors for the hoist converters to fail.
The production hoist W2 pinion brake would not release.
Elevator got hit by forklift on the bottom and bent the bracket that tells the plc that the elevator has landed at its position so the doors wouldn't open because the plc wasn't seeing it being in the landing position.
This was not a roof fall. A pot out occurred in South Mains primary escapeway between xc 76-77. This pot out was not above the anchorage point. This was reported due to the pot out being in the primary escapeway which impeded travel.
The heating unit in the office trailer caught fire resulting in the roof of the trailer burning. The structure did not burn completely down but, it did sustain serious damage.
The CM operator backed the machine out of the face due to the machine's conveyor not working. The electricians had CM operator try to start the conveyor to determine the problem. A piece of rib of what would become the No. 2 face fell out onto the CM operator. The rib measured 53"" long, 36"" wide, and 20"" thick.
Employee was putting a bearing on a mixer with hand. The bearing moved quicker then expected and ee smashed finger between the bearing and the mixer. The tip of left index finger was amputated by dr.
Employee was removing steel from hole after drilling hole, a piece of draw rock fell & hit employee's hand cutting pinky finger on left hand. Employee crushed the tip of little finger & had tip amputated. Employee returned to work on 04/21/2017.
An EE parked EE's scoop and exited to wait for the shuttle car to tram by the EE was standing between the rib and the scoop bucket. The scoop bucket was sticking out past the corner of the rib, when the shuttle car was tramming through the entry and slid into the scoop bucket shoving it against the EE and pinning EE between the scoop and the rib.
The original date of injury reflects 4/6/17; The employee has continued normal duties. On 5-31-2017, the employee went to a doctor and was diagnosed with a hernia. Description: The employee was lifting a timber onto the ride and felt a pulling/stinging feeling in right groin area.
Employee was putting up bolt and bolt bent. EE then spun the bolt around and the plate struck right forearm lacerating it.
EE was using the SteelWeld Shear to cut metal pieces to make a machine guard. When EE activated the foot pedal on the machine, EE's hand was in position under the clamp to hold the metal. EE's right middle finger was caught between the hydraulic piston that holds the metal being cut on the SteelWeld Shear work table.
Employee needed to extend the ventilation curtain and as employee exited the back of the bolter by stepping down approximately 1 foot to the uneven mine floor, employee twisted the left ankle. Employee began missing time on 5-1-17.
A fully trained miner sprained right shoulder while lifting a ventilation tube. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was removing rock from the #2 raw coal belt and felt a pain in left lower back.
Employee was exiting the deck of the underground forklift and EE's foot got caught on the back of the seat, causing EE's left knee to twist. Employee continued to work until 06/05/2017.
EE was making a screen replacement on screen #2. EE had placed new screen in place & was exiting the upper level of screen #2. EE placed EE's feet onto spring cover to place screen rail & bolts. After installation EE began dismount from spring cover onto catwalk approx. 2 ft. Rt foot slipped off, lost balance & Rt foot wedged Btwn deck & screen. Body twisted causing strain to knee.
Employee was working on a piece of equipment when a breaker was tripped by another employee flipped the breaker while the employee's hand was in the area of work(belt)and it cut the pad of the employee's right middle finger tip. Employees were not following Lock Out/Tag Out procedures.
Employee was in the cab of the scraper when employee reached around with the left hand to shut the cab door with the right hand resting on the door frame post, and closed the cab door on the right thumb.
Miner was shoveling under a feed hopper and reported pain. Miner was told by a doctor there's a hernia and will require surgery to correct the problem. Miner will be off approximately 6 weeks.
Employee was unloading timbers and one of the timbers fell on employee's foot.
Employee was lifting a 6x6x8 foot timber with another worker when the worker lost footing and employee tried to keep timber from hitting ground straining employee's lower back. EMPLOYEE HAS BEEN WORKING SINCE DATE OF THE ACCIDENT AND WAS TAKEN OFF WORK ON 11/2/17 DUE TO SURGERY ON LOWER BACK.
Employee was busting rock and felt a pain in shoulder, the next day, back started hurting.
EE was getting into a scoop when ee slipped on the metal decking and sprained left knee
A fully trained miner was injured while rib pinning when the loader operator knocked a ventilation tube down and the tube was pushed into miner's Rt. Leg causing miner to twist knee. The incident was not a result of any compliance issues nor were any protective guards involved.
Employee was carrying a 4'x4'x10' pontoon with a forklift when the bottom corner of the pontoon contacted the ground causing the pontoon to rotate on the forks contacting the driver side door. Employee put hand up attempting to block the pontoon causing a laceration to left hand.
When support column was loosened it fell and hit employee's right foot/heel causing a cut to employee's right heel.
A fully trained miner was using a grinder to cut wire mesh over their head. Miner dropped the grinder causing it to cut their Rt. wrist and received stitches. The incident was not the result of any compliance issues.
Employee was crossing the belt line with the dust hose in employee's hand when employee fell striking the track rail. Employee bruised employee's ribs
Employee was attempting to clean out Jaw conveyor tail pulley when another employee bumped the power, pulling employee's arm into the tail pulley.
Was bending a bolt to put in top and accidentally hit rotation while pushing it into the hole. Started missing work on 5/1/2017.
EE was working at the face when a piece of coal (8""x5""x3"") fell out from between the roof straps and struck EE in the left thigh. EE suffered a bruise and was taken off work.
WHILE GRINDING ON CRACK ON DISCHARGE OF DEWATERING SCREEN, GRINDER HUNG ON CRACK AND KICKED BACK; KNOCKING OFF WELDING HELMET AND HITTING EMPLOYEE'S MOUTH, CUTTING LIP.
IE was removing the rear hoist brake pressure plate on a P&H 4100 shovel to replace Orings. When air was applied the brake casting broke causing the pressure plate to strike the IE below the left knee. IE was referred to an Ortho for an MRI to determine if there was ligament damage. On 5-4-17 IE attended a follow up appointment & was issued two weeks restricted duty of no work.
Employee felt a pain in the back as EE moved a bottom roller out of the roadway.
Motor was being lowered onto vertical pump shaft, choker cable broke motor dropped down vertical pump shaft approximately 3 feet. Employee was hit on brim of hard hat caused laceration over left eyebrow and abrasions on left hand and lower left leg.
Employee was picking bundle of roof bolt plates up when ee felt something pull in right arm.
EE was stepping out of the shuttle car to change direction and twisted left knee.
Employee right thumb was smashed between hydraulic cylinder and pushing wedge. Resulting in a crush thumb.
A miner who had undergone treatment for cancer believes they broke a rib while stretching. The miner was seated in the breakroom before beginning work and raised their arms above their head, stretching and felt a tingling in their back/side. Later was diagnosed with a fractured rib. It is unclear if this was the event that caused the fracture.
Employee was pulling drill steel out of roof, when they felt a pain in their back and twisted their knee. Employee did not miss work until 05/08/2017.
Employee stated was working on the Co. No. 262 CM and while lifting the rub rail back into place to reinstall the rub rail felt pain in right shoulder. This resulted in a strain and lost work days.
Employee was disconnecting a pipe at the pumping site. As the employee removed the last bottom bolt, the pipe spun and struck the employee on employee's hard hat.
Employee was walking out of a warehouse, the heavy wind gust knocked a 12'x12' door out of the track, and it came down hitting the employee
While walking between two four-foot culvert pipes and carrying a nuclear test gauge in right hand subject, in anticipation of an uphill step, shifted weight to right leg and the nuke forward creating a rotation on the planted leg which caused EE's patella to dislocate and EE's leg to give out.
Employee was cleaning a rock off the top of a Miner in the 4 West TG section when a rock fell from the top left side of roof mesh striking EE's left hand resulting in a contusion. Roof mesh was being installed at the time of incident. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was exiting excavator and their foot slipped off the step. This caused employee to lose balance and fall to the ground. Employee went to a physician on 04/24/2017 and was diagnosed with a broken rib.
Employee was removing a piece of rock from shuttle car when employee dropped it onto feeder causing lacerations to 3 fingers on both hands.
Employee was helping remove a rock from the jaw crusher and strained back.
A dozer operator was discovered lying beside a portable toilet in the field. EMS was contacted and the employee was transported to a local trauma center. It is believed that the employee passed out after exiting the toilet and fell back hitting head and back against the toilet's skid. Employee was diagnosed with two fractured vertebrae in lower back.
Employee was exiting plant and walking to control room when fencing broke loose from corner of feed belt and material fell striking employee on nose and above eye. Employee was wearing all required PPE.
Employee was retrieving a grease gun off the 4-wheeler and as ee was leaning over foot slipped. EE sought medical attention two shifts later and was diagnosed with moderate degenerative changes. Because experience in low coal seams and the repeated trauma to knees we are reporting this as an occupational illness.
EE was loading bolt plates. EE felt a pull in groin area. EE was seen on 04/22/2017 in the ER where there wasn't a diagnosis and nothing found. EE continued to work. This became reportable as of 08/15/2017 when EE had to have surgery for a hernia and was taken off work.
While stepping over relay bar, employee stepped onto uneven bottom causing strain to right knee. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.
Employee was standing at the control panel of feeder when the feeder washed out the employee was struck in the knee and upper leg with rock and debris causing tear in knee requiring surgery.
A temp employee was hauling gob with a diesel scoop. EE reported to EE's foreman something hit EE in the back of the head while driving the scoop back to a turn around. The foreman noticed signs of concussion and initiated response. The area involved has been checked twice by an investigation team. It is believed the TE struck EE's head on the top of the seat or on the mine roof.
Employee slipped on rock in step while dismounting dozer and fell to ground.
A fully trained miner was removing a chain from a draw bar when the draw bar fell to the ground striking miners left foot. The incident was not the result of any compliance issues nor were any protective guards involved.
Getting off crusher tire, slipped, fell, landed on metal panel. Injured left upper thigh below buttock. Sore & bruised.
Employee was heading down the hill in 785C Rock Truck after unloading, road was wet after raining, lost control, started going side ways and hit a rock wall and turned over.
Employee was in the process of getting rocks out of throat of miner in the 4WTG section when a rock fell from in between the straps striking right hand causing a laceration and fracture.Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
A fully trained miner was injured when dragging a dust hose and bumped their head on a roof strap causing pain in their head. This was not a result of any compliance issues nor were any protective guards involved.
The IE was removing mud from the transmission linkage area under a F-250 pickup when a second employee, attempting to test the motion inadvertently put it into reverse and the front left tire made contact with the IE's chest. The IE was diagnosed with simple thoracic spinous process fractures.
A fully trained miner injured their back when loading material onto the belt a piece of rock fell from the coal rib striking miner in the lower back. This was not a result of any compliance issues nor were any protective guards involved.
Our maintenance mechanic was doing pre-operational inspection and preventative maintenance on a Hitachi EH 1100-5 haul truck on the morning of Wednesday, April 19, 2017. As ee was exiting the equipment ee slipped on the bottom step and fell to the ground. Landing on right side causing pain to this area.
Claiborne hauling Co. truck driver, operating truck # C-212, came onto site at 10:30 am needing to wash bed out. Preceded to wash station entered into an adjacent area and proceeded to raise bed into high voltage line.
Employee was driving a scoop and ran over a rock, jamming employee head into the canopy of the scoop injuring neck.
Employee was carrying a vent tube with another employee and tripped and fell. The employee strained right shoulder.
Injured was roof bolting in a future LW recovery chute. During the process EE had a drill steel become fouled in the drill chuck. EE was unable to pull it out, so EE then activated the rotation to free it. When EE rotated it, the steel whipped striking EE in the right upper portion of the chest.
Employee stated that after they had moved the bolt machine up to set up a new row of bolts, a section of draw rock fell and struck employee on the left side and left leg.
EE was walking down #4 entry (#1 unit) to roll up a curtain near the face when EE stepped in a hole. EE said that EE felt a pop in EE's left knee. EE has been experiencing some soreness in the back of EE's leg, but continued to work until May 4th. This is when EE went to see a doctor who has kept EE off work until the results come back from having x-rays and a MRI done.
A fully trained miner was walking under the 104 Belt when EE struck head on the belt guarding causing pain in the neck and left arm. The accident was not a result of any compliance issues nor were any protective guards involved.
Employee stated another employee came up from behind and wrapped arms around ee and kicked leg around ee and when doing so hit ee's lunch bucket into right knee. Causing sharp pain
Employee stated that employee was attempting to unload stopping plaster, when employee stood up with a 50 lb. bag of ""B- Bond"", employee felt a pain in employee's back.
Employee was stung by a bee hive.
While installing a new head shaft on a shuttle car, injured was using a sledge hammer to position shaft into place when a piece of metal broke off and went through injured's clothing and lodged beneath the skin in employee's lower left leg. The metal was removed by a doctor and the injured was placed off work for 7 days.
EE was placing a bulk bag on the unloader. It would not go up high enough to get the discharge spout over the hole to be able to untie the bulk bag. EE had untied the bag and was pushing over in the hole. EE fingers were pushed against the edge of the opening. The edge went between 2 fingers on left hand.
Employee was helping center bolt in the 4WTG section under the bolter canopy when a rock fell slid down the steel and struck employee in the face causing a laceration which resulted in sutures from plastic surgery. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
The crew was belting up and EE was on the tight side of the belt when a piece of coal rolled onto EE's back.
While moving full barrels of oil, EE managed to pinch wrist/forearm between barrels. Pinch/crush injury, no fractures.
The employee had partially walked up a berm to look for a pole location marked by a stake. As EE turned and started down EE's feet slipped on the hardened material causing EE to fall backwards onto a rock at the bottom of the berm.
Employee was moving cable near the roof bolting machine when foot became lodged and as EE turned, EE twisted right knee. Employee is currently off work waiting on an MRI.
Injured was driving pickup when going downhill after leaving worksite. Driver applied brakes and brakes where OK, then as truck got faster, driver applied brakes again, brakes failed. Drive tried down shifting and at this time made a hard left turn and truck rolled over causing injury.
A fully trained miner was injured while drilling a bolt hole. A piece of rock fell from the top striking EE's left arm causing a laceration which required stitches. The injury was not a result of any compliance issues nor were any protective guards involved.
Injured was a passenger in back seat of pickup truck and was not wearing a seat belt when driver started down the hill and applied brakes and they were OK. After pickup started getting faster, driver applied brakes again and the brakes failed. Driver tried to down shift but was forced to make a hard left causing pickup to roll over causing injury.
As operator was dismounting water truck, boot caught on the step causing knee to twist
Injured was a passenger, back seat of pickup truck. After driver started downhill the driver applied brakes and brakes were OK. When truck picked up speed, driver applied brakes again and brakes failed. Driver tried to down shift but was forced to make a hard left turn causing truck to roll over, causing injuries
Injured was a passenger in front seat of pickup truck and was not wearing a seat belt when driver started downhill. Driver applied the brakes and breaks were OK. As truck got fast driver applied the brakes and brakes failed and driver tried to down shift and was forced to make a hard left, causing truck to roll over.
Employee was installing a termination kit on an electrical cable. EE removed EE's gloves and started slicing the outer jacket of the cable with a razor knife using EE's right hand when the knife slipped contacting the right side of EE's left thumb tip causing a 1cmx1cm avulsion to EE's left thumb. Seen by site medic, referred to ER where EE received restrictions
The employee was hanging miner cable in the intake and felt a pain in groin area. EE continued to work through 4/22/2017. EE's first day missed was on 4/24/2017.
We are submitting this as an occupational illness because any condition is due to cumulative, repetitive trauma. The EE sustained a back injury at home and had previous back pain. Subsequently, claimed to experience back pain while roof bolting. EE was examined twice and released to work with no restriction. EE was examined a third time and issued a lifting restriction.
Employee slipped on a hill and strained left calf muscle.
Employee was removing a Kennedy stopping, the stopping had a mandoor in it. After removing the majority of the stopping, the mandoor rolled off of the remaining few panels and struck the employee on top of the foot. Late reporting due to recent medical evaluation placing employee on days away from work starting on 6-5-2017.
Two miners were installing a new gear onto a shaft on the Rolls Crusher. An unplanned ignition of propane gas occurred when they were heating the new gear with a propane grill and cooling the shaft with liquid propane. Equipment operator with 3 weeks experience as a re-hire and 2 years experience previously with RGI, injured employee's ribs on employee's left side when employee fell approximately 4-5 feet on a rock. Employee was attempting to ascend a track mounted Volvo excavator.
While opening airlock door employee lost control of the door. While trying to catch the door, right ring finger got pinched between the door and frame. When it got pinched, employee jerked hand back causing laceration. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.

EE climbed out of unit on EE's own power, was transported by ambulance to Hospital ER. Examined, nine (9) stitches to parietal area at top of head, released with instructions for care of wound.
Operating shuttle car. Hit a hole in the roadway and hit employee's head on the top of the cab.

Employee was assisting a forklift operator who was transporting a pipe. The pipe shifted and rolled over the employee's left hand while the load was still on the ground. The employee received left hand second - forth digit contusions. The employee underwent proximal interphalangeal joint capsular release with pinning of the joint surgery on October 31, 2018.

Employee has a good attitude for safety. EE did not have a specific incident and is believed this is a result of a body failure type injury. Employee has proper training and was wearing required PPE.
While rock dusting, employee stepped in a hold and felt a pain in employee's knee. Employee worked all schedule shift until taken off by a surgeon on May 3rd
While moving a roller on a pallet, employee felt pop in left arm.
A fully trained miner was injured when a steel plate fell off of the scoop car that miner was working on and struck their left leg. It was not the result of any compliance issues nor were any protective guards involved.
Employee was performing metal grinding operation in maintenance workshop. Employee was wearing safety glasses, full face shield, gloves and hearing protection. Employee had irritation in employee's eye upon completion of shift. Went to ER the next day, small fragment removed.
Employee was evaluating a stuck vehicle and was walking to the rear of the vehicle and tripped on a windrow of rocks. While stumbling forward, the employee planted their right foot and felt a pop in their right knee.
Employee was palletizing stone and as ee stood up after picking up a stone ee felt pain in lower back. Employee did not request medical attention for this until 4/17/17.

Employee was dismounting the cat 83005 fork lift, employee stated they slipped while dismounting the fork lift and was not able to grab the handle. Employee fell on right side of body injuring right elbow, right shoulder and back
Employee performed normal duties, on Friday 4/7/2017, of handling material. On morning of 4/8/2017 (Saturday) Employee felt pain in left shoulder. No specific incident. Employee went out for surgery 5/24/2017.
Loading 8"" x 10"" x 11"" H-beam onto a flat bed trailer with fork lift. The injured attached a chain to the beam to reposition it on the trailer. When the fork lift raised off the bed, the chain slipped causing the beam to roll hitting pinning ee right hand between the beam and trailer resulting in an amputation of the tip of the middle and index fingers.
Employee was cleaning out trailer and somehow fell and fractured wrist. Accident was reported to management on 4/10/17.
EE was moving a piece of plate steel to gain access to timber to place under load of new steel shipment. EE lifted the approximately 3' x 5' by 3/8 inch steel the long way and pushed over to the side 180 degrees. The steel plate hit an approximately 18"" high steel beam and kicked back striking the EE's heel causing EE's heel bone to break in hairline fractures
While setting up our crane (we were hired as a contractor for crane service) an employee of our own, got hand in the way of swivel block of crane while assembling the crane. Broke middle and right ring finger on left hand.

Employee was trying to start the LT-58 Lo-Trac and could not. EE opened the engine cover to see if the breaker was knocked, the engine cover came down on EE's right index finger causing injury to the tip end of the finger.

A contractor was operating a shuttle car on the unit 2 section. In the process of operating the s/c EE hit a ledge in the mine floor causing EE to strike the canopy of the s/c with EE's hard hat. EE filled an incident report but did not say EE was going to the doctor which refereed EE to a specialist. EE rtw on 4/20/2017 notifying the mine sight EE was off due to a strained neck.
EE was walking from EE's car to the bathhouse when EE was hit by a truck and suffered a broken right leg. It was a snowing and visibility was bad. The driver of the truck said EE did not see the victim.
EE was bolting in #3 heading around 1:15 pm. EE was attempting to push a roof bolt into the top when ee's bolt wrench kicked out of drill pot and struck ee's left hand.
Employee identified a broken crusher door pin at the rail loadout. After employee and the electrician removed the broken door pin, the employee held the crusher door in place while the electrician left the crusher platform to get a new door pin for replacement. The door then fell on employee's foot.
Tripped over a rock or the fuel hose, EE was not sure which one. Landed on right knee on frozen gravel. EE was limping and scratched knee. EE immediately cleaned with antiseptic and covered the wound.
Employee was lifting blocks to put under D9 dozer blade. Employee heard a pop in their arm.

Employee was operating a Volvo A35D Articulated truck when the truck slid in a tire rut. The operator had right hand on the wheel and left hand on the grab rail. The sudden movement jerked employee's body to the right causing pain in left shoulder.
Employee was exiting a diesel truck. As they stood and twisted to exit the employee felt something in their L thigh. Employee was sent for diagnosis. Dr.stated no treatment needed. EE reported pain persisted. Sent for follow ups. EE missed first day of work on July 5, 2017.
When driving Maintenance Vehicle back from mining section, vehicle hit a chunk of salt in roadway. Steering wheel turned fast and caused strain on right shoulder and arm. Arm kept bothering employee. Went to doctor, and doctor wants to do surgery. Started losing time from work on 11/13/2017.
EE was walking up to miner to start bolting when EE slipped on an accumulation along the rib that caused pain in left knee down. The EE continues to work but started PT on 04/12/2017 and is now a NDL accident.
On road tractor trailer driver was dumping the limestone from the quarry at the processing plant and the tractor trailer rolled 90 deg. on its side.
Employee was loading a 2"" water pump onto a mantrip and felt a pain in lower back.
Employee was helping hang miner cable when ee felt pain in left groin area. This resulted in a hernia.
Dismounted from the operators deck of an underground mining machine, and stepped down onto the mine floor, at which time employee twisted/rolled left ankle.
An employee fueled 609 backhoe. While mounting the machine employee felt a pop in right shoulder. Employee was evaluated by a Dr. on 4/2/17 and was not released to return to work, making this incident a lost time.
Employee doing first of the month task,(checking fire extinguishers). The employee was climbing to the deck of a 980K loader to check the fire extinguisher. While on ascent the employee hit their left knee injuring it.

Employee was changing bits on the E28 longwall shear. EE was on the head gate side of the shear and attempted to rotate the drum with another employee. EE pushed on the drum with right foot and felt a pain in left groin. Diagnosed with a hernia.
Shipping operator loading out of silo 22 had EE's left pinky pinched between the silo spout and the platform steel when EE tried to manually adjust the position of the spout for a poorly-aligned truck. After getting help from a maintenance employee, the spout was either accidentally raised or shifted into the beam, pinching the operator's finger.
Employee was processing stone and went to lift a piece of stone and felt a pain in their lower back. They refused medical at time of event. Employee went to doctor on 5/8/2017 and doctor put them on light duty.
Employee was drilling holes in steel plate with a 1/2"" shank drill. The bit got hung up and twisted the drill out of hand. The cord for the drill became wrapped around the trigger causing the drill to continue to twist and it caught right pinky finger.
Employee was kneeling on the deck lid of a belt transfer chute that was being installed. EE was attempting to remove a pin that was stuck in the adjustable legs of the chute. As EE jerked real hard on the pin, the pin came out of the hole and the employee lost balance and fell to the ground resulting in a fracture of right arm and a hairline fracture to pelvis.
Employee stated they lifted miner cable and water line and stepped forward and felt a sharp pain in the right knee. Employee returned to work on 4-28-2017 on light duty. On 5-1-2017 employee was placed off work under doctors orders.
Employee states while shoveling rock they hurt their shoulder. Employee notified the supervisor on Friday 5/5/2017 that the injury occurred on 4/27/2017. Employee currently off work due to restricted activity.
After EE had been working on hands & knees EE was walking up the stairs and felt and heard a pop from the right knee with immediate pain and it hurt to put weight on the right leg
Employee was assigned duties as a shop mechanic. Employee was installing a tire on a haul truck and claims that they injured their back while performing above mentioned duties. The Mine Operator is contesting that the claimed injury occurred on the mine site.
Employee was attempting to reposition the leg of employee's jackleg machine by lifting on the D handle with employee's left arm when employee felt a pop in employee's upper forearm area near the elbow followed by pain. Was seen by site medic and referred to the ER where restrictions were issued.
Employee was attempting to remove an empty 55 gallon steel drum off an eight inch tall secondary containment pallet, slipped and fell onto left side.
Employee was walking down a stairway at the Bulk Scales and slipped and fell down 4 or 5 steps. Stairway was clean, free of any trip hazards and well illuminated. The employee injured pelvis area.
Working with employee from the mine removing dog bones from a 988G machine to take to shop for repair. Dog bone fell forward hitting and fracturing employees right arm
Employee was driving Jeep, turned into 3 South commons and door came open. Employee reached out to catch it and got the left index finger caught in door hinge.
Assisted with removing door pins & door shifted pinching/crushing left index & middle fingers between washer & mill base. Both fingers fractured & both nail beds significantly disrupted. Surgery performed w/ both fingers the distal phalanx was pinned with K-wire & repaired both nail beds with small suture.
The EE was hammering in pins on feed chute, while hammering the pins the EE struck the pin and EE's hand at the same time. The pin caused a laceration that required stitching to close. The EE was taken off work for two days and placed on modified duty after that.
Employee stopped forklift to pick up a piece of broken plywood that had nails to throw it away. Employee unbuckled seat belt and stepped forward with right foot and felt a loud pop in knee.
A Master Mechanic was servicing a D8 Dozer on the morning of 4/22/17. As the Master Mechanic stepped down from the tracks to the blade bar, right foot slipped on mud that accumulated on the bar, causing left foot to twist inward.
The miner slipped and fell on EE's back and rolled 3 to 4 feet. The miner had right shoulder pain. Original diagnosis of right shoulder sprain on 4/24/17. The miner had no restrictions/full duty and has had no lost time from the accident. On 7/7/17, diagnosis changed to a torn rotator cuff and is scheduled for Right Shoulder Surgery on 7/12/17.
Injured was helping construct a structure at an outby location, and fell and hit the ground with left side and lower back (tail bone area). Injured complained of pain in back and was taken to hospital for x-rays. It was discovered with the x-ray that the employee had cracked one of employee's ribs.
While running the Stone splitter to make Dimensional stone. Employee laid right hand on the power feed conveyor/roller table that you control with a handle. Employee then engaged control that makes conveyor/roller table move forward and table pulled EE's hand in between 2 rollers and wedged EE's hand in between the 2 rollers temporarily and bruised hand severely.
Miner fell from highwall 14'6"" to the lower level. Miner suffered 5 broken vertebrae and laceration to the back of head. Miner was ambulatory after the fall.
Employee was assisting in the removal of the gear box from the #1 Sand Dryer. Two bolts that were holding the gear box in place broke, causing the gear box to fall onto the employee's leg and causing a left proximal fibula fracture.
EE strained their shoulder 4/20/2017 while replacing rigging pins (75#) on the bucket of D33 dragline. EE continued to work and did not report the incident until 5/10/2017 and did not seek medical attention until the next day. EE was taken to the doctor and received no restrictions but EE was scheduled 6 therapy sessions that started on 6/5/2017 making this a recordable injury.
Employee was greasing excavator boom and slipped on the tracks. Employee hit employee's head on the tracks/ground and received a concussion.
EE was walking on an access platform of a mill ball sorter. The ee missed a step and caught self with left arm to avoid falling. The ee felt a pain in left shoulder. Employee sought medical attention the day of injury and was released to full duty. After a follow up evaluation with a orthopedic on 5/8/2017, the employee was placed on restricted duty.
Miner was exiting a front wheel loader. As boot touched the ground, miner twisted right ankle and broke right leg.
EE was removing shovel cable from front of light vehicle. Operator of vehicle moved forward & made contact with the EE who was removing the cable. EE didn't fall as a result of the vehicle but as the vehicle proceeded forward & was still pulling the cable, the cable made contact with the EE which caused EE to fall. EE sustained multiple contusions.
Employee sustained a fractured right tibia when foot caught an e-stop cable while stepping down from L-357 conveyor dust collector structure causing EE to fall awkwardly onto the walkway.
While employee was lifting an oxygen tank from the ground to the truck, the employee felt a pull in their back.
Small drill operator operated in an area against a small high wall in an unsafe manner. Upon setting up the drill operator placed their back against the high wall at the drill operator's station. Several rocks (approximately 15-20lbs) became dislodged from a height of 2'-3' above the operator's shoulder height striking operator at the base of the neck and shoulder.
While cutting conveyor belt with razor knife, employee slipped and cut wrist.

Working on CV 17. When entering the HSI crusher EE slipped and fell, EE tried to catch self and hit EE's shoulder. Then was help by EE's coworkers by making sure EE was ok and called mine supervisor before transporting EE to main office.
Employee was adjusting deflector gate on 1 South screen on Monday 4/10/2017. On 4/12/2017, EE noticed that EE's abrasion was more painful and started to swell. At 2:00 pm I went our clinic with employee to be seen by our doctors. Doctor advised employee to continuation of EE's anti-biotic and Dr wants EE to be seen on 4/14/2017 to check EE's infection.

EE #1 was cutting C-Channel steel to remove it, while standing on a 6ft A-Frame ladder while EE #2 supported middle on another 6ft ladder. A piece of slag hit EE #1 causing EE #1 to turn, temporarily blinding EE #2 who stepped off ladder. EE #3 stepped forward to see if EE #2 was okay & steel broke off, fell 10ft. glancing EE #3's hard hat & hitting their shoulder.
A Miner was attempting to uncoil cable from a suspended spool, when it became disconnected from its rigging, striking the Miner. The Miner sustained neck and leg injuries
Team loaded excavator onto trailer for transport. Employee climbed to top of equipment to get a height measurement for permitting. EE was not wearing fall arrest gear. While climbing back down, EE slipped on some hydraulic fluid and fell approx. 16' to the ground. EE's left arm hit the trailer beam, EE bounced off the track and landed on the ground.
An employee's right foot was pinched in between a diverter plate and the grating. The employee was transported to Banner Churchill Hospital and was released back to work. Due to continuing pain, the employee was seen on 7/12/17 and it was determined that they needed to work on restricted duty.
Employee was walking on the walkway near the cyclones in the Utica I dryer building and tripped on a step. Employee braced themselves with the left hand as they were falling and fractured the left wrist. The front of the step was painted yellow and a caution step sign was directly overhead.
Employee was installing track ties in a bad section of track and strained employee's back. Employee started missing work on 4/13/2017.
On 4/07/17 employee reported a strain/pain to right shoulder area while pulling lever down in load out area of #14 Plant and refused medical attention. Later on 06/16/17 same employee reported pain still present and taken to local Urgent Care for observation and diagnosed with strain of muscle to right shoulder.
Conveyor belt stopped due to material build up. Employee went to clear material and belt rolled backwards. This caught employee's hand and pulled the arm into tail pulley. This broke employee's forearm. *After investigation it was determined the belt tightened back onto the roller after material was removed which trapped EE's forearm.
Employee was pulling shovel cable from cable reeler, when cable snapped at splice and hit employee knocking ee to ground. Employee dislocated right knee and had surgery on 04/21/2017. EE began losing time on 4/21/17.
Employee was bending over sliding drill on forks of our lower and strained employee's lower back.
Employee went to sit down on a guard and it gave out causing employee to hit their head and elbow on the wall behind them.
While attempting to slide the counterweight of a Rotex machine into place, the weight shift landing on the associates hand. The resulting injuries were lacerations to the left hand that required stitches to close.

Reached out from operator's seat to wipe debris from bucket attachment area - struck control and was struck by loader arm.
EE working on Komatsu WA1200 Front Loader. EE was installing an intake coupling (or intake boot) between an air box and first piece of intake pipe. EE experienced pain in right thumb. Right thumb has torn ligaments. Two prescriptions, a splint, and work restrictions were given at the clinic. Surgery is required to reconnect ligaments.
Employee was installing a split set bolt with a jackleg and pushing bolt in with a 2' driver. The chuck tender on drill came open and the driver came out of the drill and fell down striking employee on the right ring finger which caused a fracture to the finger.
EE was walking off the catwalk and miss placed foot.
Plant Operator reached under a vendor-owned tank trailer that was being offloaded to pick up an empty paper bag that had fallen to the ground. After retrieving the bag, the Operator raised up and arm contacted a sharp edge on the trailer's fender, cutting the arm above the left elbow. Operator received five stitches to close the wound.
Employee was tying in lifter and rock came off face, hit ee in the middle of the back and received a thumb sized hole about 1/4"" deep.
EE was attempting to loosen a nut with a wrench. Nut was diffucut to break loose EE felt pain in the back. EE was taken to ER X-rays were taken. ER reported Muscle strain of the upper and mid back with possibility of rib factures.
An employee was tasked with changing a screen and EE was hammering a bolt into place EE felt a pain in EE's right shoulder. While exiting the screen deck EE grabbed onto the spray bar to pull self out of the screen and felt a pop in the same shoulder. EE was brought to the clinic and diagnosed with a sprained shoulder. EE was put on restrictions and physical therapy.
Employee was placing a sledge hammer into the bed of a pickup truck. while placing the hammer in the bed employee lost control of the hammer and the handle swung up and struck employee in the left forehead.
Mine operator was backing up a 915 load haul dump with forks after setting a piece of face conveyor. As the operator was backing up the corner of the cab caught on a two inch lip on the rib, which caused the cab to break loose from the machine, pushing the operator against the steering wheel. The employee sustained injuries to the chest, left shoulder and left arm.
Employee was using a Dewalt 6"" grinder to cut/fabricate a piece of metal for support for a welding machine. Grinder kicked back and struck left hand. Injury was to the left hand middle finger, ring finger, and forefinger, just above the palm. Employee was sent to Lee Memorial Convenient Care where EE received 13 stitches to the fingers mentioned above. Restricted Duty prescribed.

Employee stated while tramming roof bolter to #3 entry from #4, employee was backing out of the #4 entry, the monster mats hit the corner rib, causing them to slide off of the top of the lids and struck employees right hand. Employee was placed on restricted duty 4/29/2017.
Employee was preparing to replace some belting on the stacker. The conveyor needed to be rotated, so employee used a pry bar to rotate the pulley to get the belt seam off of a roller. The bar slipped off the tail pulley and jammed wrists from the shock load of slipping. This caused a contusion to the left wrist resulting in suspected fracture of capitate bone.
Employee strained neck when the haul truck ee was operating slid into a rut on the road.
Employee stated employee was walking in #4 entry Inby the feeder line and employee rolled employee's ankle causing employee to fall onto the mine floor.
Operator stepped off of the loader ladder and the floor was not even and they twisted their ankle.

Miner was using a Tamrock drill rig and was attempting to remove the drill bit by hitting it with a hammer. Another hammer was propped against the drill bit to secure the bit. When EE struck the bit with one hammer, the other hammer slid down pushing EE's finger across the broken drill bit which lacerated the finger.
Employee hit hand on beam while holding end of pipe causing laceration to right index finger requiring sutures.
Employee was clearing a chute wall when build up slid and pinched employee's left foot against chute opening
EE reports that they first swung a hammer and struck right hand pinky finger against metal frame of conveyor and then shortly after that EE reports that while using a socket wrench it slipped off the bolt and they struck the same area on their right hand against a belt clamp.
Operator injured left forearm. The straps on a pallet of empty bags had been cut but not removed completely. EE tripped on the straps and fell forward. EE tried to catch the fall with right arm to the ground and by grabbing the handrail on the nearby steps with left hand. EE missed the railing and left forearm contacted the steps about 3"" below the elbow.
Employee was operating the RS-20 while standing on shield #61, as EE lowered shield #67, a rock measuring 12""X 12"" X 12"", fell from between shields 67 & 66. The rock landed on the cable tray and slid down the face to shield 61 where it fell off striking EE in the right ankle, resulting in a hairline fracture to the ankle. EE was released to modified duties.
The employee was operating a man lift when ee struck a protruding roof bolt pinching finger between the roof bolt and the hand rail.
Employee was using a magnetic drill when the bit broke, causing the mag drill to spin loose and smashed employee's right thumb. INJURY BECAME REPORTABLE ON 5/30/17 WHEN EMPLOYEE WENT BACK TO DR AND NEW XRAYS SHOW A BREAK - RESTRICTIONS BEGAN ON 5/30/17.
Employee was using a jack to put a rail locomotive back onto the track and the jack bar struck on the side of EE's head resulting in EE getting 13 sutures.
Employee was cutting a small piece of belting and pulled the knife in to employee's right leg above the knee. Making a 1 inch cut
Employee was raking clinkers off the kiln #3 grate when ee felt something pop in left shoulder. The pain comes and goes. Employee reported the injury on 04/28/2017 and was taken to the doctor on that date.
The IE was working in an engine compartment of a 795 haul truck when ee lost balance. When ee grabbed the deck of the truck to catch balance the twisting motion on shoulder caused it to pop.
EE failed to disconnect the 4 inch hose causing to be sprayed with sulphur on legs.
Employee was using a 5 lb sledge hammer and poke bar to clean material build up in the tower. While striking the poke bar with the hammer, the hammer head slipped off of the head of the poke bar and the employees right point finger was crushed between the handle of the hammer and the head of the poke bar. The finger tip was fractured and stitches were used to close the wound.
The employee was grinding on cracks underneath the water truck. The employee was using a 6-ft ladder placed near the back tire to reach the undercarriage of the truck. The employee stepped off the ladder onto the berm area and rolled the left ankle. The employee sought medical attention and was given a rigid boot and restricted duty for one week.
Ee was holding a hydro blaster in right hand and was trying to move the control pedal with foot when ee accidentally stepped on the pedal causing water to penetrate hand through two places. Seen at the ER by Dr, x-rays negative, ee released to restricted duty, wound keep covered, but open to drain.
Ee was pulling on hose and it broke free, striking ee in the mouth. Seen at the dentist, diagnosed with 3 broken/chipped teeth, one of which had to be extracted. Ee released to restricted duty for two days.
EE slipped into a gator hole which was covered by vegetation while walking during the performance survey duties, causing knee to impact the ground and/or twist.
Employee was changing retract cable on shovel and cable struck employee on the shoulder resulting in one day of restricted duty.
Operating loader and hit a pot hole bruising employee's sciatic nerve.
Employee was stepping down off of fuel trailer using left foot & left foot rolled. Diagnosis: left ankle sprain.
Employee was in the process of removing bridge from crusher throat. A rock shifted and temporarily pinched EE's left foot. EE was able to kick off the rock and remove self from the area. EE's foot was bruised.
The employee was putting a return roller on the conveyor under impactor and felt a strain in hip/leg area.
Shoveling out the rock and grease accumulation around the bucket boom of a 992K and felt discomfort in forearm. Still had movement. Sent for examination. First doctor returned employee to work with no PT. At a check up a second doctor recommended PT on 4/20/2017 with sessions starting 4/21/2017. At work on light duty.
On 4/19/17, employee requested medical treatment, was placed on restricted duty and was prescribed physical therapy. The employee did not use safe means of access on a screen and fell 3 feet. The physician diagnosed a L-hip sprain and contusion in addition to lumbar radiculopathy.
After exiting the cab of the underground drill the operator walked on uneven ground and rolled their left ankle. Ice was applied on site. The next day the operator went to the doctor and was placed on light duty for a sprained ankle.
Employee was attempting to cut a hole in conveyor belt, using a box cutter type knife. Employee accidentally cut the tip of employee's right middle finger.
Injured worker was performing clean-up at the tertiary duct. EE was pulling a bucket of loose material out of the duct with a rope, when ee slipped and fell against the handrail on the scaffold platform hitting lower back.
Employee was working on the discharge bearing of the ball mill when EE's left thumb was caught between the swivel and the bearing cap. The employee returned to the clinic on 4/24/2017 and was given restrictions.
Employee was filing the feeder on the portable plant with oil in the tank. Step out of feeder enclosure and left knee popped.
While carrying a pressure washer, employee slipped and fell on knees and hands. Diagnosed with fracture of thumb on 5/3/17.
Employee was engaged in steel erection activities and was installing tube braces with two other employees. They attempted to push the brace into location and it shifted back and pinched the employee's finger between the steel.
EE felt pain in right shoulder while traveling mortar product overhead in a newly constructed cyclone plenum. EE didn't seek medical treatment believing it was just a sore shoulder. EE returned to Wear Con facility on 5/8/17 where EE sought medical attention and was diagnosed with a right shoulder muscle and tendon strain of the rotator cuff and place on restrictive duty.
Emp. was changing a belt and caught a finger between the sheave and belt. Emp was treated onsite and returned to work that shift. On 5-1-17 Emp complained of pain and was taken to clinic where a small fracture of the tip of the finger was found. Incident became reportable at that time.
EE was operating a hand winch adjusting the dredge position. EE felt hand get weak followed by a sharp pain. EE was taken to a doctor where the hand was x-rayed and wrapped. EE was released to go back to work at normal job with restricted use with EE's right hand. EE was released back to full duty one week later.
The Employee was stepping out of the CAT 259D Skid Steer and felt a strain to the right lower back.
Employee stated was rolling up extra curtain and cut thumb on a curtain hook. Note: Employee did not receive stitches but was given light duty
Cleaning coal tunnel, shoveling coal out of trough. Employee took a shovel scoop with a hand shovel and ee felt pain lower back.
Employee was throwing bags of trash into the dumpster and felt employee's left shoulder popped out of place.
Employee complained of pain in employee's left rear during employee's shift operating haul truck.
The crew replaced gaskets on the turbo hose on the drill motor. Once drilling recommenced, smoke came from the area of the turbo hose and exhaust system. IP obtained an ABC dry chemical fire extinguisher and discharged on area of smoke, causing inhalation of dry chemical powder resulting in irritation of EE's respiratory system.
While stacking chopped stone onto a pallet, a piece of stone fell off the pallet and landed on EE's right foot causing a small contusion on the top of the foot.
Employee stated EE was laying rail on 1 Section when EE felt pain in stomach. Employee later sought medical attention on 4/15/2017 and was placed on restricted duty. This incident will be reported as restricted duty.
Employee stated ee was driving wedges when setting a feeder. Employee struck left index finger with the hammer. The blow caused a blood blister which later became infected. Employee was placed on light duty on 4-16-2017.
Employee was working on a loader when the part EE was working on shifted and tweaked EE's back.
Employee stated EE was operating the shuttle car on the #1 Section when the shuttle car went over an irregularity in the roadway, jarring EE. This resulted in a neck strain and restricted work days.
Employee was climbing down from loader and fell off ladder landing on ground. Bruise to wrist and shoulder requiring restricted duty status
Employee was driving haul truck and where the road switches from gravel to asphalt, drove over a pothole which resulted in a discomfort in abdomen. Released to modified duty for 3 days.
Employee was walking through the coal tunnel when EE's foot slipped and EE felt a pop in the knee. We sent EE to the Dr and EE returned to work but was not allowed to squat. EE could perform EE's normal job and did until Dr later found a torn meniscus. Contacted OIEI in Denver and was advised to report when EE goes to surgery. LTI started on June 1st.
Employee was operating the scraper when ee hit a pot hole and felt a sharp pain in back. Employee called the supervisor and notified ee of the incident. The employee was taken to the clinic where ee was diagnosed with acute lower back pain. Employee was given prescription medication and restrictions.
Employee was working on a spillage chain, had to get in the housing. Once inside had to bring a heavy piece of channel iron in the area to complete the repair planned. Employee had no injury on 4/8 and had no symptoms. The employee reported the alleged incident on 04/11/2017.
Employee was walking to the employee building. Employee turned to left to go back inside the shop and slipped on some loose gravel spraining right hand.
The employee was dressing the edge of 1/4"" thick diamond plate on a stationary grinder. Employee was grinding the 3 1/2"" wide end holding the piece with both hands toward the top of the material. While grinding the piece of material employee's hand was pulled into the grinding wheel causing a laceration on the top of the left hand.
Employees were replacing a pump casing, using a hammer wrench which got stuck on a bolt. As one employee reached to try to remove the wrench, the second employee attempted to hit it with a hammer. That employee struck the other employee's thumb, injuring the nail bed. Injured employee was given restrictions.
Miner was climbing into a IT-28 wheel loader and missed step. As employee started to fall they caught themselves with right arm and felt a pop straining right shoulder. No medical treatment was requested at this time and a report was made. On 4/11/17 miner requested to go to the doctors and per doctors request was placed on restricted duty on 4/11/2017.
Employee states that ee was walking to oil the baker cooler in the basement of 3 & 4 herreshoff through 6 to 8 inches of spilled material and stepped in a hole under the material.
on 04/06/2017, the EE in question reported that they had hyperextended their right knee while relocating a pump but continued working unaffected by the incident. It was later determined that the EE had torn the ACL and Meniscus of the right knee. EE reported to work on restricted duty on 06/08/2017 following corrective procedure on the knee.
Adjusting width of forks on FL068. Employee was standing on right side of fork, facing the outside of fork. Using both hands, applied pulling pressure toward self with both hands moving fork away from backboard slightly. EE was not aware finger was between fork and backboard. As employee released fork with right hand, it came down pinching tip of left index finger.
Employee was pounding out a roller set from a frame and hit left fifth finger. On 4/10/2017, employee was diagnosed with a fracture and placed on restrictions.
Employee was getting loaded at the 46 shovel. When the first dipper was put in the truck employee experienced some jarring in the seat. The employee was seen at the clinic and released to return to work. On 4/10/2017, employee returned with continued pain and was given restrictions and returned to next regular schedule on 4/13/2017.
Employee was operating scoop tractor in belt drive area of 2 RT RMS. Work area was 37.5"" in height and 18 feet in width. While positioning the drive with poor visibility due to mine conditions causing rubber tire of the machine to catch and push up 2"" waterline along side the operators compartment striking employee on right side of head driving safety glasses into right eyebrow.
Dredge operator tripped on swing cable while stepping from dredge ladder to pontoon. Employee fell to the deck of the pontoon suffering a sprain/strain lower back.
Prior to start of shift on 4/5/17, employee reported a pain in their neck & was sent for medical evaluation. Employee operated a bulldozer the prior day to spread clay on a stockpile. The employee stated that no incident or accident occurred on 4/4/17 before noticing this pain. Employee later described turning their head to back the dozer as a contributing factor.
The employee was carrying a Screen eccentric on shoulder, while taking it off shoulder to load in the back of a pickup truck ee smashed middle finger on right hand between the tailgate and eccentric.
Employee was assisting a stopper drill operator. The stopper drill slide/tipped toward the employee striking the employee on the back side of the right hand. No fractures, swelling, or bruising was observed. Employee was placed on light duty to allow hand to heal.
While flipping the belt over on the beltline, slipped on tail roller, fell and landed on thumb.
Lab operator lifted a tote weighing 15.5 lbs. and twisted at the waist not pivoting feet and felt a pull in lower back.
On the #1 section, roof bolter was in # 7 entry. A piece of rock fell from 10 feet high. The rock was 3' long by 2' wide tapering to 4-5 inches thick. Rock fell at the front edge of ATRS. Employee turned to get away and the tapered end of rock struck employee's left heel.
Employee stated was pulling a fly pad off of a board to move up. When employee pulled on the fly pad there were loose rocks laying on top of the board. A rock 13""X9""X1.5"" thick fell striking employee on the right shoulder.
Employee was stacking bags from a bagging machine to a pallet. As the employee stacked a bag, EE felt a pop in EE's back. EE initially thought that EE had just turned the wrong way. After stacking two more bags the pain increased the employee reported to the supervisor and requested medical care. EE was placed on restricted duty.
Employee was holding onto a piece of 3"" tubing while another employee was striking it with a sledge hammer. The sledge hammer glanced off a bolt head and hit employee holding the tubing in the wrist. The incident resulted in a broken wrist.
Employee was walking up and down the preheater tower continuously for several days causing pain in EE's left foot.
Employee was assisting install a new set of air lock doors on the 20 HG longwall section. EE set the jack against the mine roof and was preparing to adjust the set screws to make the jack secure firmly to the mine roof. In the process of preparing to tighten the set screws the jack slipped and slid down smashing the employees left hand and finger.
Employee were doing road maintenance with a hot oil pull behind tank trailer when an explosion occurred. Employee went to doctor on 4-18-17. Placed on restricted duty.
Employee was pulling out cable dip stick on excavator and frayed wire impaled left index finger
Employee was turning belt and shiv by hand to access set screw to remove bushing. As ee turned the belt, left forefinger was clipped by the belt and shiv. This resulted in a laceration and finger tip fracture.
Employee cut (non-gloved) thumb on left hand with razor knife while trying to widen pre-drilled bolt hole of belt skirting resulting in 4 stitches to close open laceration.
Miner was using a utility knife to cut a section of 6"" hose from a flange fitting. Utility knife slipped resulting in a laceration to left ring finger. This required 8 sutures to close.
Employee was doing repairs to a drive belt motor. A bolt was removed that allowed the motor to pivot and swing from its position. When the motor swung, it hit employee's hand and smashed ee finger between the motor and the housing. EE was wearing gloves but received a laceration.
EE was working on a feeder. After getting the feeder on the tailpiece, EE was trying to place a pin in the feeder to hold it in place. While operating the controls, EE was attempting to place the pin in the hole and got left index finger caught in between the pin & the frame of the feeder cutting the tip of the finger. EE had to have 4 stitches to close the wound.
Employee was setting a spool piece into a coal transport line when the spool pinched the tip of ee finger (right 5th digit). The pinch resulted in a fracture of distal tuft.
While removing a flight from a loader conveyor chain, a piece of metal flew off the flight when struck with a sledge hammer and hit the employee under bottom lip. EE received two sutures.
EE was driving a mule and felt material get in their left eye. EE flushed out eye and requested a medical evaluation. EE was given a prescription.
The employee set a crib block on employee's finger while building a crib. This caused a laceration to employee's finger. Sutures were required to close the wound.
Employee stated that EE had taken the Surf Yard truck to retrieve EE's harness. EE stated EE stopped truck outside portal & got out to pick up EE's harness which EE stated was inside portal. Employee stated EE walked behind truck to retrieve harness & noticed truck beginning to move forward. Employee stated EE received injuries while attempting to stop truck
Employee was replacing tickets in the ticket box. When EE turned to go back to the scale house, EE slipped and fell on rocks, causing laceration to left knee requiring 7 stitches.
EE was climbing down a ladder from the belt press plant. EE was going too fast and slipped on one of the steps which caused EE to fall the distance of 6-7 steps onto a concrete floor. EE hit the back of their head, right knee and wrist.
Employee was loading bolter supplies onto Bolter when a rock (2ft. X 2ft. X 1 inch thick) fell from roof striking employee in left forearm making a 1 3/4"" laceration that needed 5 stitches during treatment.
The employee was in the process of hooking up a pull behind rock duster to the three wheeler. The attachment tongue of the duster slid and caught EE's right index finger between the attachment tongue and the ride.
EE was falling a tree and was hammering a wedge into the tree and when ee hit the wedge it flew out of the tree and hit ee in the nose.
Installing roof bolt when EE spent the bolt next to the top & small piece of material went into EE's eye, EE had safety glasses on at time of accident. EE went to eye doctor & a piece of metal was removed from eye. There is no lost time.
Employee was burning out a keyhole bolt in 071-12 pan feeder on the west side of the discharge chute. The employee stopped to chip out dirt/slag out of the hole using a chipping hammer. After employee struck the hole in the liner the slag/dirt ricocheted behind their prescription safety glasses.
A maintenance mechanic got an infected eye from exposure to Calcium Oxide (Lime).
Employee adjusted the grizzly bars at the primary crusher and failed to close the access door before the feeder started back up. While closing it a rock fell onto the door and injured EE's finger, requiring two stitches.
employee was driving to the unit to repair a continuous miner. EE traveled thru a clear curtain on the supply road, as ee traveled thru ee didnt pay attention to the road and struck a coal rib.H EE was seen for laceration to left arm and left ring finger that required stitches.
Individual had a piece of metal fall and hit hardhat. It then cut the back of EE's neck. Six stitches. No lost time.
Employee was struck in the left hand by a piece of metal that broke off of the pick hammer employee was using while installing a 6"" hugger onto a main water line.
EE was shoveling dirt away from a hopper using a wood handled shovel. EE was not using gloves that were provided to ee and a wood splinter went into hand which got infected because ee signed a form to waive medical treatment on the day of injury.
Employee got left foot caught between the first and second step of platform used to shoot kiln ash ring causing employee to fall onto the burner floor causing a laceration to left thumb.
Ee was loading in shot hit a rock, loader stopped suddenly jamming left wrist into the controls. Ee was examined by Fit for Work therapist was treated with ice, bandage wrap, exercises. Checked several days over week. Ee stated still had some pain. Sent Ee to clinic on 5/3. Ee returned on 5/4, restricted no manual transmission. Moved Ee to operate plant, hand/wrist not used.
Employee was loading a bottom roller onto #2 man bus, while setting a roller down it slipped out of employee's right hand smashing left little finger between the roller and man bus.
GMS employee was retrieving a bag of rockdust near the rear of the bolting machine and the rear of the bolting machine was lowered causing injury to the employees foot.
Contractor was helping to move supply doors which were located on 20 HG A belt. They freed the doors and were removing the masonry block from around the doors. Several blocks were hung along the doors where the waterline and cables went through the block. In the process of taking down the doors one of the blocks fell and struck contractor on the right hand smashing a finger.
While operating haul truck, employee got jostled in cab when running over pothole. Later, employee began to experience pain in lower left hip that radiates down the left thigh. Employee was diagnosed with sciatica and referred to chiropractor.
Caught employee's left arm in the steering wheel of a sweeper truck causing a hair line fracture.
Changing a tire on an articulate truck while breaking tire down from rim with a hydraulic ram the ram kicked out under pressure and struck employee in the face.
Windshield wiper repair in progress. Turned wiper motor on to line up pin on sprocket with linkage. Sprocket rotated and pinched middle finger between the linkage and housing. Employee disregarded company policy and training on lockout/tagout procedures. Employee received 16 stitches to the left middle finger. No impairment, released for normal duty as a driller.
Employee was filling in on a roof bolter when a piece of draw rock fell & cut right shoulder. EE went to ER received 4 stitches. No loss time, full duty return to work.
A fully trained miner was installing a beam when miners right hand was caught on the beam causing a laceration to miners finger that required 8 stitches. The incident was not the result of any compliance issues nor were any protective guards involved.
Installing metal floor grating on the raw mill, witness was sliding grate into position and didn't realize employee that was helping guide had hand in a pinch or mashed position. Causing the grate to mash left hand between the grate and floor frame.
The incident occurred while washing a work truck with a high pressure water cleaner located at the mine operator's vehicle washing facility. While washing the truck, water under high pressure exited a section of the hose, passing though rubber gloves, resulting in an ""injection"" injury to the individuals right index finger.
While performing maintenance on rotary dryer wind blew sand up under EE's safety glasses into right eye. EE was taken to local hospital where eye was flushed out and prescription was given.
Employee switched out with the other roof bolter operator when a piece of drill steel came apart, bounced and hit the drill pot when the steel hit the employee in the mouth which resulted in tooth being chipped (fractured).
Employee was walking in the No. 8 Entry when they came in contact with a curtain hook striking them behind the left ear. This resulted in a laceration requiring 11 sutures.
EE was removing pusher bolts from the crusher. Using left hand to guide the impact wrench to remove the bolts, the left index finger got caught between the lip of the crusher, and the impact wrench fracturing the finger. EE was taken to the ER. EE was diagnosed with an internal fracture of the left index finger between the first and 2nd knuckle. On light duty.
Employee was conducting a pre-inspection of a dozer and was climbing off the machine when employee slipped and fell on the left hip.
Employee was cutting a piece of cord lash, when the utility knife slipped & cut EE on the inner part of the lower left thigh. EE went to ER, received 5 stitches and returned to work full duty on the following workday 04/24/2017.
Loader operator activated fire suppression system after flames were noticed and suppressant got in operator's eyes.
As employee crossed Altapass road to enter Plant 1 dust was blown into right eye. EE entered Plant 1 and flushed eye to remove dust. Then on 4/27/2017 EE reported to supervisor that EE thought eye was scratched. Management took the employee to eye doctor and had eye examined. It was found to be scratched but no particle found in eye.
Employee was using a small disc grinder and accidentally allowed the grinder to come in to contact with leg resulting in a cut that required six stitches to close.
While attempting to remove solid concrete blocks from the raw coal crusher chute employee reached in to remove a block and another block fell striking employee on the hand, cutting right ring finger.
Employee was making sure Drill Press drill bit was securely tightened. Right hand slipped causing contact with the drill bit and employee cut finger on right hand needing 3 stitches.
Employee was trying to cover an electrical panel with a tarp during a storm. The employee pulled on a rope that was attached through an eyelet on the tarp to secure the cover. The eyelet ripped out of the tarp causing the employee to cut self on the left arm with a knife the employee was holding in right hand.
While installing roof bolts in the first row of the No. 8 Right cut, employee was drilling when a roof bolt plate became lodged between the supply tray and the drill boom causing the roof bolt plate to be ejected striking the employee on left cheek. Resulted in a laceration requiring 5 sutures.
On 4/20/2017 an employee was walking on a sidewalk on the ports property and misstepped and fell. On 5/26/2017 the employee reported they were having discomfort with the knee and went to the doctor. On 8/10/2017 the doctor determined that the discomfort was caused by the fall. Physical Therapy has been prescribed. This did not happen at the Mine site.
EE WAS WELDING HANDRAIL WHEN A PIECE OF METAL FRAGMENT ENTERED LEFT EYE. EE WAS WEARING SAFETY GLASSES AND WELDING HOOD.
Miner was attempting to clear a jam on the conveyor belt. Miner performed LOTO on the equipment and attempted to adjust the belt by grabbing with hands. During the belt adjustment, the miner heard and felt a pop in the middle finger on right hand. Possible strain or misalignment to the finger. Miner has not missed any work due to this injury.
Employee was trying to remove a new pair of scissors from packaging. The scissors were attached with zip ties. In contrast with company policy the employee used a razor knife without cut resistant gloves and while attempting to cut the zip tie EE slipped and cut hand resulting in 10 stitches.
Employee was under shaker holding the nuts for another Employee using an impact gun to tighten when pieces of dirt fell and went into the right eye causing it to irritate the eye
The employee had just taken off glove, the pin had grease on it and the end had a rough edge from being hit with a hammer.
EE and co-worker were working on the tail pulley shaft for the mainline stacker. They had taken a come along and attached it to the shaft of the tail pulley and pulled the shaft backwards. When EE went to remove the bearing from the shaft the bearing turned and crushed EE's finger in between the bearing and the tail pulley adjuster.
Employee was tightening a bolt on a rotary feeder when the socket slipped and the right pinky finger came into contact with a metal edge causing a small laceration.
Employee was walking through curtain carrying tank of oxygen, as EE went through curtain EE smashed right pinky finger against rib.
Employee was hooking up a manifold and got hit in the mouth by the hose causing tooth to come out.
While attempting to remove an oversize rock, with two other workers, from the #2 conveyor belt the miners left hand was struck by the rock when it began to move between the belt and rail; resulting in the miner receiving sutures for a laceration.
Alleged: EE stated on next shift that EE got something in eye when needle scaling the previous shift. States self flushed and was wearing safety glasses and face shield at time of event. On 4-20-17, EE was evaluated in the ED and given rx medication for eye.
Employee was disconnecting a chain from a pump case, Second employee was grabbing the jib crane hook to move it out of the way as employee readjusted, the hook slipped from hand and swung towards the injured employee contacting injured employee in the mouth which caused a 1/4"" cut to the right upper lip.
The miner was helping the certified electrician splice A 300 volt D.C shuttle car cable. The miner placed the cable on employee's leg then cut the insolated jacket of the cable with A razor knife when the knife cut through the cable causing A laceration to the miners leg.
EE was walking to screen room floor from truck side. Another employee was opening the door from 3rd floor and ee tripped and broke fall with left hand, cutting middle finger on a broken hinge. EE was given stitches.
Employee dropped a beltline down to splice the belt. Employee pushed the belt slack with left hand, while pushing employee stepped backwards onto a 4"" lip in the mine floor. Employee lost balance and fell onto the left hand, this resulted in a dislocated finger.
Dark rainy conditions with poor visibility road way was bumpy due to recent rain events. Employee was in haul truck when EE hit a bump in the road the bump bounced EE in the seat which EE had aired up quite high. EE is a tall person and the bump caused EE to strike head on the roof of the truck causing EE pain in the neck and the right arm.
Employee got debris in eye during preshift inspection of water truck.
Employee using a slide hammer to drive pin out of a shearer ranging arm. Employee got left little finger caught between the hammer slide and driver causing a cut that required 9 sutures. Accident caused by the employee's lack of attention to off hand. Employee didn't violate any rules, policy, or laws. Employee has a positive attitude for safety and was wearing proper PPE.
Hammering on a rock, the rock broke and a piece flew up and hit employee in the forehead
Employee was taking guard off scoop charger w/help of another employee. They were trying to lift it off when injured employee dropped EE's end catching EE's 3rd finger on left hand between guard & charger. Went to ER received 2 stitches, return to work next day. NO loss time.
Contract employee was cutting the belt for making a mechanical splice, as EE was cutting the belt, the knife contacted upper leg when EE reached the edge of the belt resulting in a laceration. Cause of this injury was body positioning. Compliance with company rules was a factor and possible miner proficiency. Mining equip and/or systems & PPE not a factor.
Employee was picking up lid on PC 79 and smashed left hand causing laceration.
Miner was clearing hard buildup from an air slide flow gate when the gate closed while hand was under it. The gate pinched fingers causing a cut to right 4th digit that resulted in stitches.
Employee was trying to get away from a bee that was in the crusher tower when EE was stung by the bee above upper lip. Employee was taken to NextCare urgent care where EE was given two shots in case of allergic reaction. Employee was released to full duty on 4/14/17 with follow up visit on 4/16/17.
Employee was removing rust from the #8 chute when some rust landed on EE's eyebrow and safety glasses prior to getting into EE's right eye.
Miner received a laceration on left pinky finger, for proper healing 5 stitches were required. Miner's finger was pinched in a guard access door while changing a bearing.
Injured was carrying T channel along the side of bolting machine with partner when they hit a previously installed roof-bolt and the T channel bounced back hitting injured person in lip with sharp edge and cutting lip. Injured needed stitches.
Employee was installing grizzly bar on the down river crusher. EE picked the bar up with an electrical hoist. When putting the bar into place the EE pinched right thumb between the grizzly bar and the frame of the crusher. This caused the thumb to burst open and caused a laceration that required 8 sutures.
Employee was using the pointed end of a pick hammer to punch a drain hole in a 5 gal. hydraulic oil can. Employee's left hand was on top of the can supporting it. As employee took a horizontal swing at the can employee slipped causing the hammer to glance off the can, striking employee's left hand.
The EE was installing a wing shroud on a dozer. While attempting the pound the keeper pin in place, the ee hit the pin and glanced off hitting left index finger with the hammer. The ee was sent into the clinic and received two sutures. Risk 1. Low
Driver was disconnecting unload hose from pump on tanker using manual force and under no pump pressure. When hose dislodged, employee struck self in the upper lift with the fitting on the end of the hose causing laceration to upper lip, resulting in 5 stitches to lip.
Climbing on equipment frame to check for leak. Slipped off equipment frame and hit right knee on edge of equipment frame causing laceration.
The operator missed bottom step on 2012 Kawasaki loader. EE fell and hit elbow on step & needed 2 stitches.
Holding pully and left thumb was pinched
Employee was attempting to pull the monorail trolley down with the diesel tugger. The monorail kinked causing the trolley to lock up. The tractor pin connecting the tugger's nylon winch rope to the trolley broke. A clevis struck the injured employee in the right hand causing a laceration requiring stitches.
A fully trained miner was struck in the lip when installing a hose on a longwall shield. The hose struck ee in ee's lip resulting in ee getting two stitches. The incident was not the result of any compliance issues nor were any protective guards involved.
Employee was inserting an auger into the box of a highwall beam and got left index finger caught between the auger and box.
Employee had finished drilling a hole and had inserted a tube of resin and a bolt into the hole. As EE was pushing the bolt up to set it, the hinge pin for the drill mast fell out allowing the mast to lower and tilt to one side. The drill mast contacted a drill steel in the steel tray and flipped it up and struck the EE in the left ear, requiring stitches.
Employee was hanging drill cable, turned around, hit a roof-bolt plate on the right side of their face requiring ten (10) stitches.
Employee was roof bolting on the #9 unit when the pinner steel fell from the roof and struck the boom and hit employee in the mouth causing small laceration to the lip and chipped a front tooth.
Employee stated was removing bags from the east transfer point dust collector on the fourth floor of the lime shed when the employee removed goggles fogged up. Employee removed them for cleaning and lime fell into the employee's eye. The eye doctor used a prescription to treat eye.
REPORTABLE ONLY DUE TO SUTURES - Employee was operating shields when a piece of rock came over the handrail hitting the lower right leg causing laceration requiring sutures.
JRAY truck driver was using SafeRack loading platform. EE raised the platform using the lever instead of the rope and pinched finger between the lever and a rock straining basket that was sitting on the platform. Two fingers on hand required stitches.
An employee strained lower back while moving exploration samples. Employee saw a doctor on 5/1/17 where employee received manipulation treatment.
EE was walking in the Pellet Plant. EE tripped on a chain jack, marked with barricade tape, that was attached to the floor. EE fell and struck right knee. The ee was transported to the Clinic for evaluation and received sutures.
An employee raised their face shield and residual grout dust fell into their eye. The employee rinsed their eye and continued to experience irritation. Employee was medically evaluated and treated for a corneal abrasion on 4/12/2017.
A Shop Mechanic was trouble shooting the airflow through the radiator fan when employee's right hand fifth finger came in contact with the fan blade. Resulting in a laceration requiring seven stitches.
While turning over a wall panel for the coal mill bag house with a forklift, the wall panel slid off the fork and pinned employee against a tool box. The panel hit around the lower side of employee's left leg.
Employee was attached a 4' section of 4"" threaded pipe to a 90 degree pipe el and it slipped when the threads did not catch and part of the threaded portion struck employee on the left wrist.
Employee was working on equipment when debris got in right eye.
Employee attempting to loosen the chuck on a cordless drill that had a hole saw bit in the drill & as EE was twisting the chuck their left hand slipped causing #2 & 3 fingers to contact the teeth of the bit resulting in lacerations. Seen by site medic, referred to the ER, 2 sutures were required, Full work release was issued making this a medical reportable.
Employee was installing a high voltage plug in the back of the section power center. EE was having trouble tightening the plug all the way and EE struck the high voltage plug wrench with a hammer to assist in tightening it. EE accidentally hit hand between the thumb and pointer finger. EE received two stitches.
EE worked on Saturday in the tunnel welding and moving metal pieces and felt something in right eye but didn't want to go to our work comp Doctor, EE went on own to EE's own Dr and didn't turn it in until EE received the bill. EE had a metallic foreign body in the right eye.
Descaling Kiln Shell w/Air Hammer. Reported Rt hand hurting which brick had fell on earlier, wanted to quit for the day, Sat. am, 4/08. We received call noon Tues. 4/11. ""fractures to Rt Hand"". getting treatment. Med Asses: ""Closed fracture base of 5th metacarpal bone Rt hand."" Splint placed. Return to ""Modified Work""--no impact tools w/Rt hand. Also referred to Orthopedic Dr.
EE reported lime burns on the toes of EE's left foot. EE was assisting the Maint. Dept. with lime-fines silo repair. EE stated EE noticed issues with EE's foot a couple of days earlier, but continued working until reporting it on 4/8/17. EE's physician prescribed burn ointment and scheduled a follow-up appt.
EE and a co-worker were changing a motor mount on a Chevy pickup. EE was underneath the truck and the co-worker was using a pry bar to remove the mount. The co-worker dropped the bar and it struck EE in the mouth. The impact chipped one of EE's front teeth and caused a small laceration to the underside of EE's tongue.
On 4/12/17, employee requested medical treatment and was prescribed chiropractic therapy by a physician. The employee alleged that ee hit head, while wearing hard hat, on a cross beam while walking under blade mill 1A in the process of greasing the plant equipment. EE was diagnosed with cervical and thoracic spine strains and was returned to full duty.
Employee was drilling holes for bolt insertion and the drill kicked striking EE in chin.
Employee oiling a 4.5 inch air grinder with a cut-off wheel. While setting the grinder on a work bench, the wheel spun and made contact with the employee's knuckle on right index finger. The grinder cut through EE's weld glove and caused a laceration. The employee received three stitches.
While cleaning a ditch at front gate the employee must have tweaked right knee. Employee is unsure what happened & when it happened, there was sudden pain, leg just started hurting. Employee has arthritis in right knee that rubbed together and damaged the meniscus with a flap tear. Employee had out-patient surgery on 5/19/2017 to repair meniscus and they removed arthritis.
Employee was using left hand to hold plow bolt head while turning nut with impact wrench. Bolt spun in left hand causing deep cut requiring 19 stitches.
Miner was walking outside, through plant, windy day. Lime dust got in left eye. Miner was wearing safety glasses. Was given a prescription for antibiotic.
Was working on dump truck and crawled out from underneath, and felt a little pop in left knee.
Miner was walking through plant and dolo dust got in right eye. Miner was wearing safety glasses. Miner received a prescription for an antibiotic.
The truck the injured employee was operating, was being loaded and dust came through the window that was rolled down halfway, and employee felt dust go into left eye.
Employee setting tailpiece, while turning turnbuckle w/hammer the hammer broke loose striking EE in the upper lip. This resulted in laceration requiring 2 stitches to the lip. EE went to ER @ Lonesome Pine Hospital in Big Stone Gap, VA. EE had no loss time and has full release to RTW.
Employee was in the process of removing a continuous miner bit from the bit sleeve. As EE struck the bit with a hammer, EE struck index finger/hand in the process causing a laceration to finger. EE received 4 sutures as a result of this injury.
Employee was hanging a high voltage cable when employee fell from the ladder employee was using striking the mine floor. This resulted in an injury consisting of 3 cracked ribs.
Employee was assisting another employee to set a feeder on the unit 3 section. In the process of the employee standing helping to set the feeder the manifold blew off of the 8 inch metal water line. This caused water to hit the employee and for the employee to slip hitting the rib with left elbow. When elbow struck the rib it first struck the corner of a rib bolt plate.
EE was replacing a blade on a razor scraper when the blade broke and lacerated their left thumb, requiring stitches to close the wound.
EE used brake cleaner and Krol oil to clean a shaft. The product pooled in the key way and when EE inserted the key; the product shot up into EE's eyes. The chemical went under EE's safety glasses and prescription glasses. Reportable only due to prescription medication for eyes.
While working on cable reel on shuttle car, lid covering reel fell and hit EE's hand. EE went to ER and has a broken little finger and received 9 stitches. EE is off work until EE sees orthopedic doctor.
Employee was assisting to make hydraulic/mechanical repairs on a roof bolting machine drill head/mast, at which time another employee functioned a hydraulic control lever which caused movement of the machine and injured the employee's left thumb.
Employee was in the process of installing a roof bolt, as ee adjusted the boom on the bolter, a roof bolt from the bolting tray came in contact with a hose on the boom, the roof bolt struck the employee in the mouth causing a laceration, ee received six sutures.
Hot stick slipped off rabbit box resulting in the handle of hot stick hitting employee on the bridge of the nose.
Was putting cable behind anchor. The shuttle car lost power. Went and put power back in, and the stop, start switch was stuck and the cable started taking up and hit employee on the back of the legs and flipped employee. Employee landed on left wrist and head bounced off the bottom of the mine floor.
While cleaning K-121 belt in the cooler pit, a temp employee noticed a troughing roller between 4 and 5 kiln coolers that was stuck and not turning. EE starting clearing material away from center roller with a shovel. When the roller broke free, it caught the shovel causing finger to be pinched between the shovel handle and the conveyor structure fracturing pinky finger.
While turning the master key switch on haul truck, employee hurt right wrist. Brace was issued by doctor.
Worker was picking metal out of concrete feed. Felt something in left eye.
Employee was getting off excavator and came into contact with a sharp piece of metal requiring 6 stitches.
Employee was drilling a hole for a roof bolt. when a small rock a came off the steel and hit the pot of the bolter bouncing off and struck employee in the left ear. That resulted in a laceration that required two stitches.
Employee was changing a dual windshield wiper. EE was loosening the wiper arm from the broken stud. The wiper arm swung back and hit the employee right ring finger.
While removing pinner steel from the chuck, a piece of material fell striking EE's fingers and mashing them against the pinner boom. EE's right pinky finger had to have two stitches.
Miner was performing a gear change out on the Raymond Mill inclined grit auger. Miner was using a small sledge hammer to assist removal of gears when their hand struck an adjacent guard constructed from expanded metal. The miner received a laceration on the back of their hand that required sutures.
Employee received foreign material to eye while standing under a fender of the water truck and using a hammer to tap on a socket in preparation of removing the fender.
A helper was installing a ground rod for a light plant on a new drill site when EE inadvertently miss struck the ground rod causing EE to contact lower left lip/chin on the ground rod.
Employee was moving steel parts and received a laceration on finger.
EE was changing out bret-by on the shear carrying two pieces at a time from the head drive to the mid-face. While getting through the head gate shields with the bret-by ee fell forward, leg stayed between the shield foot, causing a pop along with pain in right knee. *Became medical treatment due to surgery on 5/26/17*
EE was working along stage loader at the LW when a piece of slate fell out from the rib and struck the employee in the left wrist, nose, lower body and head. The employee continues to work but started PT on 04/10/2017 and is now a NDL reportable accident.
Employee was using a portable grinder and accidentally had a metal sliver get in eye. The employee was wearing safety glasses and a face shield at the time of the injury. EE had sliver removed at the local hospital.
Employee was working on a Longwall shield. Employee pulled a staple out of a valve fitting and was struck by the valve in the forehead resulting in a 1 inch laceration.
Employee was changing hydraulic hose on scaler using a pry bar when the bar slipped and struck employee on lip causing a laceration.
Metal shard broke off while hammering striking employee in left leg.
Employee states that ee was hammering on a tail bearing to remove and replace. As ee was hammering, a piece of metal broke off and flew under glasses and in to eye.
EE was working on a continuous miner using a come-a-long to pull the CM cats back together. While doing so, the come-a-long came off. The hook of the come-a-long flew back and hit EE in the upper lip. EE had to have 5 stitches to close the wound.
EE was accessing crusher off the catwalk next to the crusher. EE stepped up approximately 6"" above the side of catwalk onto the crusher frame. Right foot slipped off causing the right foot to (wedge) between the frame of the crusher and the catwalk frame. This action caused the right foot to slip/twist sideways causing the right ankle to break.
This employee was diagnosed with Occupational pneumoconiosis on 2/1/17 we received the notification on 4/28/2017.
The employee felt pain in their right wrist while roof bolting. The alleged carpel tunnel syndrome injury occurred 04/28/17. The employee continued to work until they went out for surgery on 09/18/2017.
Employee reported feeling numbness in hands and wrist after hand scaling. Medical treatment on 12/14/2017 resulted in light duty.
The mine received notification on 4/26/17 that this retired ee has been awarded 8.06% for PPD from noise induced hearing loss. The ee was enrolled in a Hearing Conservation Program and was trained in use and the areas where hearing protection is required. The incident occurred because the ee failed to properly use hearing protection throughout mining career. *EE retired on 4/25/15.
EE diagnosed with Carpal Tunnel Syndrome. Surgery dates are pending at this time. EE will remain on full duty until surgery date. EE will be released on restricted duty at that time.
Quarry Stone Marker enrolled in company Hearing Conservation Program showed a Standard Threshold Shift in hearing over the previous year relative to baseline audiogram, loss >10dB.
