A roof fall occurred at/or above the anchorage point on 6 North belt line at 19 block. The fall does not impede passage or obstruct ventilation. It is approximately 8' wide X 16'long X 5 1/2' high. No other 50.20 criteria applies.

Our facility experienced a power surge causing the power to trip in areas underground and on surface. The Tiley Brake Regulator (TBR) unit in #1 hoist tripped at this time. This caused the #1 hoist to be down for 30+ minutes until it could be reset. The unit was reset and the hoist was running again by 1:45am.

During morning hoist inspection it was noted that the hoist would not respond to control commands. Investigation indicated that the computer that controls the hoist had stopped communicating with the hoist. The computer was rebooted and reconfigured, the hoist re-activated, checked and returned to service by 6:04am.

Hoist tripped out while being parked. Investigation determined a faulty sensor was altering track limit readings. Sensor was replaced and hoist checked and returned to service by 16:00 that same shift.

A roof fall as occurred at spad # 350 (9) breaks outby #3 head drive in the #4 and #5 entry crosscut. The fall measured 20 feet wide, 80 feet long, and 8 feet to (0) inches thick. This area will be rehabilitated.

The elevator went down at 6:25 AM. The safety dogs were catching at 416 feet. Adjustments were made to correct the issue.

A roof fall was found on 3/26/17 at XC 13 entry 5 8L #6 travel-way. Fall was in a four way inter section that had been supported with 8' resign bolts. Fall was approx. 20' high X 20' long X 10' no pro or injury resulted. There are no plans to clean up the fall.

An unintentional roof fall occurred in the Alma Seam, North Mains track entry, at the No. 6-7 X-cut. The fall was approximately 70 feet in length, 18 feet wide, and approximately 6 feet high.

While hoist was being inspected and ran thru its pre shift checks it had a brake failure and lost the sinking bucket 100 to 150 feet down the shaft with the hoisting cable.

Elevator door would only open approximately 2 inches, not allowing employee to exit. Employee was entrapped until approximately 5:45pm. MSHA was notified at 5:13pm

A non-injury fall has occurred in the No.6 entry of the Southeast Submains at Cross-Cut No.11, one break inby Spad NO.7943. This fall measures approximately 5'thick x 25'long x 20'wide and extends into the 6R Cross-Cut. This fall does not impede travel nor does it interrupt ventilation.

On 3/24/2017, at approximately 1:45 am, a non-injury incident occurred. The operator of the C/N 620 Haul Truck was traveling on the haul road when ee had a personal medical condition that resulted in the truck overturning. This report is being filed under the direction of the local Mesa Field Office, per J. Summers, MSHA Inspector.

A roof fall was discovered at 9:40 pm at crosscut 16-21. The fall is approximately 10' high, 25' long and 16' wide. The fall did not affect any travel ways or ventilation. The fall has been posted on both sides.

At approximately 11:48 AM, F27 Longwall intersected an uncharted gas well between 35&36 shields at 79+05. *Accident Code for Inundation was chosen because there is no option for uncharted gas well.

A fall of roof occurred in the 1st North East Mains at crosscut # 16, entry # 6. The fall measured approximately 18 feet wide x 18 feet long and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts, with 8 foot supplemental roof bolts.

The dry ridge elevator was not operational due to a faulty car safety switch. It was replaced.

At approx. 11:48 am an unplanned roof fall was discovered reportable on #3 belt, at the 1 block intersection. The fall was approximately 6 1/2' high, 16' wide and 30' long (into the crosscut). Fall was result of gradual degradation of roof strata, and possibly the roof support material itself.

Around 7:02 pm on 3/20/17, the E29HG crew experienced a face ignition while mining. Conditions contributing to the face ignition include: a clay vein traveling across the face which emitted methane. MSHA also cited bad bits, two broken tubes, and two sprays out in the throat of the miner.

Fall occurred in the #4 Entry of the #7 crosscut of the 10left section due to geological conditions. Fall was Approx. 85 ft. long, 16ft. high, and 17ft. wide. Fall area was bolted to plan with screen and 8ft. tension rebar and 16ft. passive cable bolts. No people, equipment or ventilation effected. Alternate escape way was rerouted from area.

A ROOF FALL AT THE AREA OF THE HIGH WALL WAS DISCOVERED IN THE FAN ENTRY. THE ROOF FALL WAS APX. 18 FEET WIDE X 5 FEET LONG X 12 FEET HIGH. THE FALL DID NOT AFFECT PERSONS NOR EQUIPMENT BUT DID RESTRICT THE AIR FLOW. AN ACTION PLAN WAS SUBMITTED, ACKNOWLEDGED AND THE FALL CLEANED UP. ARCHES WERE INSTALLED FOR ROOF SUPPORT THROUGH THE AREA. VENTILATION WAS RESTORED.

An Examiner was firebossing the mine and found a Roof fall at 9:20p.m. in the #5 entry at spad # 13+30. The fall was in a fourway intersection headed to MMU 001 at 6 Head (2nd East Submain) 21 x-cut.

A roof fall occurred in the 3RWL #2 entry, #7 x-cut. Seven foot resin bolts initially installed when developed. The fall dimensions were approximately 20'x 20'and 10' high. No persons affected by fall. Electrical and communication cables re-routed. Escapeway re-routed. Re-habilitation work proceeding according to MSHA approved plan using 12'cable bolts.

A fall of roof occurred in the 2nd Left Panel off the 5th SE Submain, Crosscut 56, Entry #9, at Station 33+80. The fall measured approximately 18.5 feet wide x 18 feet long and the thickness is unknown.

A roof fall has occurred on the 9L Longwall panel overtop of the crusher and stage loader area. No injuries have been reported as a result of the roof fall.

A roof fall occurred above the anchorage point in the 6 North Parallel air course at 6 block in #2 heading. The fall does not impede passage, and the air is not affected. The fall is approximately 16' wide X 30' long X 12' high. No other 50.20 criteria applies.

Hoist operator was hoisting in auto when the hoist stopped with the South skip loaded at the bottom bc of a brake fault. The operator reset the fault and attempted to lower the loaded skip. After several attempts the operator called for help. It was found that the operator did not realize that the South was in the bottom and was attempting to go the wrong direction.

A roof fall was discovered on the Old 2 right panel off of 1 north near spad 1629. The fall measured 7' in height, 19'.5"" in width and approximately 70' in length. The fall extended from 5 right to 6 right. No equipment or employees were involved. No ventilation controls were affected. Area was dangered and timbered off.

Found an unintentional roof fall on the 001 #3B Beltline at Break #13. Fall was in the intersection and was approximately 20' wide by 20' long.

At 10:15 am a roof fall was discovered in the intersection at cross cut 53 on West main. The roof fall is approximately 10 feet high and above the anchorage point. This is a non-traveled intake and no miner, equipment or ventilation controls was affected.

A reportable roof fall occurred on this day on the MMU-003 section. The fall was located on the 1-Right Panel inby crosscut #28 in the #4 entry. The fall measured 20' by 20' and was 10'+ in thickness. The area was re-supported and mining continued.

Employee was going up in the elevator to third level of production unit to report to employee's control room. While elevator was moving upward, it stopped unexpectedly, trapping employee in elevator.

3 contractor employees were using the Packhouse elevator when it became stuck. The contractor employees were out of the elevator at 11:22am.

Three contract miners were reported stuck in the Process passenger elevator between the first and second floor. The on duty supervisor called our contract elevator service company and the miners were out in approximately two hours later. Communication was maintained. No injuries sustained.

A roof fall occurred in the 1B5 Panel, cross cut #32, entry 8-9. The fall is approximately 18' wide and the height is unknown.

A methane gas ignition occurred on the H-2 section in the #3 entry approximately 20 feet back from the face.

The elevator went down at 1:00 PM. Lake Shore was called to come help fix the issue. They had to back the guide wheels loose and re-align all guide wheels on cage.

While # 3 hoist operator was doing a full test trip, the load cell system failed to work. GL Tiley was called as well as MSHA. After investigation, GL Tiley raised the set point of the system up to 7,800 lbs and achieved the load cell fault every time. #3 was back up and running at 12:52am (3/9/17). No evacuation because no one was in the mine at the time of this incident.

An unintentional roof fall was found on 3/8/2017 approx. 10:00am. The fall was in the right return between 166xc and 167xc, just inby survey station 4345. The fall was approx. 50' long and 6' thick. Timbers were set and both approaches to the fall were dangered off. No injuries occurred as a result to this accident.

Unplanned Roof Fall at xc#52, #4 entry station #6835 on the Main North belt. Fall occurred in an intersection, 4-way supported with 8' T.R. bolts. Fall dimensions - 17' wide by 30' long and 10' high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed 4-25-2015. Reported to MSHA at 4:55 AM on 3-8-2017.

During #3 hoist test trip, the load cell did not trip as expected. Supervisor decided to evacuate the mine via the chippy. MSHA and GL Tiley were contacted. GL Tiley discovered that the slack rope was not being bypassed in the system which caused the load cell to not trip. They made an adjustment and tested it twice successfully. Hoist was operational and running at 11:35pm.

Smoke was observed coming from the West property line. It was determined that a fire was present along the public road and encroaching onto the berm of the property. The fire originated from a power line being blown into a broken tree limb during a wind storm. The fire department extinguished the fire, however it was not extinguished within 30 minutes of discovery.

A roof fall occurred above the anchorage point in 4 West Intake Air Coarse at 20 block. The fall does impede passage and ventilation is not effected. The fall is approximately 16' wide X 40' long X 12' high. No other 50.20 criteria applies.

IGNITION - Occurred while cutting the bottom chain on the No. 1 Longwall face chain when methane was ignited. The flame was orange and blue in color, lasted approx. 30 seconds and went out on its own. One fire extinguisher and a bag of rockdust was applied to the area.

A roof fall occurred on the retreat mining section briefly catching the MRS unit. MSHA was contacted, an action plan submitted and the MRS was retrieved. There was no one injured and ventilation was not affected. The MRS was not damaged.

Sunday night, #3 hoist skip was found to not sound right. It was noticed that the brass slider on the back side of the dogs was twisted and bent causing the dog to bind and not retract. The mine was evacuated and the dog replaced. A full shaft inspection was done prior to send people underground, no damage was found to the guides.

03-03-2017 our elevator would not go past 240' we called FK Lakeshore to look at it. They made adjustments to the safety dogs and ran the elevator to make sure it was ok. This happened at 12:12 pm and the elevator was released at 7:34 pm.

During warm up of the kiln, there was an unplanned ignition of fuel. The ignition destroyed the cooler roof. Two employees were in the area, but no injuries occurred as a result of the ignition. The cause of the ignition has been determined as an excessive amount of unburned fuel in the kiln. The flame on the burner went out to drafting issues in the system.

Apx:1:45pm noise heard from Mainshop from wash-bay Supervisor walked over to wash-bay building to confirm, at 1:54am walked over to safety department inform Safety of Hotsy chimney was blown off. I had left and was 6 miles away. Back approx 2:08pm confirmed shut down of propane, no damage to building or injuries 2:24pm called MSHA. Appears to be failed coil.

An employee entered the elevator at the second floor to go to the seventh floor of the pre heat tower; however, the elevator stopped just before the third floor and wouldn't move up or down. Contract elevator service company called and the employees was out of the elevator after approximately 1.5 hours. The issue was a bad reverse phase relay.

Employee was operating a Cat 963 Track Loader and backed into a 4 ft. hole. The loader rolled over in the hole, there was no injury or damage to the machine.

A roof fall occurred in the 3Lt face while mining and when the roof fell in the cut it pulled one roof bolt that was located in the last row. No one was injured and no equipment was damaged.

An unintentional roof fall occurred in F28 TG Miner Section in the #1 entry just inby 25.5 wall from spad #63+38 to 63+11. The fall measured approx. 8ft high by 27ft long by 16ft wide. There was no injuries as a result of this incident.

Two employees were going up to the burner floor in the freight elevator. The elevator stopped and they were stuck in the elevator. Our contract elevator service company was called and responded quickly, but employees would be in the elevator longer than 30 minutes; therefore, MSHA was called. Total time in elevator was 45 minutes. Communication was maintained. No injuries.

A roof fall was discovered at 2 west submains parallel at spad # 4249 where miners do not normally work or travel all approaches to the fall have been dangered off at this time.

During the weekly examination, a reportable roof fall was found in the established travelway. The fall occurred in F Seam, South Main Intakes, entry 2, crosscut 17. The fall is estimated to be 20' wide, 20' long, and 6' high.

Struck by falling object

While changing work cutter head bits on the coal auger machine, ee was struck by a rock approx 4 feet by 5 feet by 30 inches that fell from the bottom section of the highwall. EE was pronounced deceased at the scene.

Caught in, under or between a moving and a stationary object

Approx. 8-8:30 pm after shift, employee drove a vehicle onto mine site and stopped close to generator, left vehicle running, gear selector in drive position, did not set emergency brake or chalk wheels. Got out and started generator. EE appeared to have come out of generator and stopped in front of vehicle to urinate, back to vehicle. Vehicle rolled, pinned EE against generator.

Caught in, under or between collapsing material or buildings

The decedent, an o/operator, was located behind the truck, behind the raised dump trailer when a load of sand dumped from the trailer. The decedent was buried in the sand. Mgmt became aware of the missing driver at approx. 10:45 am and promptly began a search. EEs began digging in the pile of sand behind the decedent's truck and the decedent was discovered at approx 11:43 am.

Caught in, under or between a moving and a stationary object

Employee stated was operating a scoop and encountered a communication and junction box hanging from the mine roof. Employee used left hand to hold the cable away from the canopy while operating the scoop. Employee's L index, middle, and ring fingers were caught between the canopy of the scoop and the rib, resulting in an amputation and lost work days.

Caught in, under or between running or meshing objects

The employee was talking to a mechanic about a problem with the skid loader. Mechanic was finishing up the repair and heard a noise. The employee went to the west side of the wet saw placed left hand on the gantry travel rail resulting in hand being run over by the gantry travel gear.

Caught in, under or between a moving and a stationary object

Worker was moving a rock on chopper table into chopping position. But the operator did not make sure it was clear before engaging the chopper action. This resulted in the loss of a finger tip on the right hand.

Struck against a moving object

Working on a fan on hydroseeder, hadn't put fan cover back on, checking fan, slipped and fell. Hand slipped into fan, catching finger tips.

During low light and heavy rain conditions, employee was walking from parked personal vehicle to the employee building. Second employee drove into parking lot and hit first employee with the mirror of vehicle, knocking ee to the ground.

Struck by flying object

Individual was splitting stone, wearing proper protective gear. Small piece of stone entered between face and the protective eye wear, striking eye. Employee cleaned eye with eyewash. Eye continued to bother employee. Co-worker took employee to medical facility for evaluation. No foreign object was found. Eye drops were prescribed. Employee did not work the following day.

Miner was manually adjusting the tines on a forklift, while letting go of tine with left hand before removing right hand, gravity took over, resulting in the right middle finger being pinched by the tine and tine backboard.

Contact with hot objects or substances

Employee sustained burn to right ankle when a piece of slag fell into a crease in jeans on top of boot trim; slag burned through the material and dropped down into the boot while performing welding repairs. Physician placed employee on 'off work' status.

Over-exertion in lifting objects

Employee was lowering ventilation tubing from the mine roof when EE felt a pain in left shoulder.

Employee was loading concrete blocks into a scoop bucket when employee felt a sharp pain in employee's lower back. Employee was able to continue working until employee started missing work to have surgery on May 31, 2017.

Employee was climbing down from dozer. The wind blew the employee down. EE fell on the push arm and was pinned for a few minutes.

Struck by falling object

Employee was removing an ice chunk off of 10b conveyor and it slid off contacting employee's shin. Fractured tibia bone.

Employee was operating a hand held grinding tool. The grinder bounced back off the metal surface cutting the employee's left hand.

Caught in, under or between a moving and a stationary object

Ee was rebuilding an air slide. EE was drilling holes through a cloth membrane, and holding the cloth in place with the other hand. The drill broke through the fabric and caught glove on the other hand, pulling it into the drill bit. Seen at the ER, diagnosed with an avulsion, and put off work.

Struck by flying object

While reclaiming belt structure a piece of waterline broke and struck the employee in the eye who was standing 20 feet away from the waterline. Employee was given restrictions that do not allow employee to fulfill employee's normal work activities.

Struck against a moving object

Employee stated employee was riding the #865 mantrip to the section with the rest of the crew when the mantrip hit a hole in the roadway jarring employee's back. This resulted in a strain and lost work days.

EE was walking on a catwalk and carry come-a-long and right foot slipped and ee slid on catwalk landing on left knee.

Struck against a moving object

EE was operating a jeep on track haulage when the jeep went off track causing the EE to hit head and left upper back. The continued to work till see by a Doctor and was diagnosed with a broken rib on 3/31/17 and taken off work.

Over-exertion in pulling or pushing objects

Employee stated was pushing finisher into hole that had just been drilled. It hung up and employee felt something pop in lower back. Employee did not miss any work until 04/10/2017.

Fall onto or against objects

EE slipped on the feeder of the crusher while crossing over it to prep the side plates for repair.

Struck against a moving object

EE was injured when motor pushing flatcar uphill lost traction and slid outby striking other motor.

Over-exertion in pulling or pushing objects

EE was barring down clinker in the preheater transition chute when ee felt a ""pop"" in right elbow accompanied by numbness in hand and fingers. EE was treated by a physician for an injury/irritation of the ulnar nerve and placed on restricted duty pending clearance to return to full duty.

Fall to the walkway or working surface

Pushing wheelbarrow tripped on piece of coal - right ankle strain or sprain original injury but broken fibula and ankle was medical diagnosis.

Fall down stairs

Employee was walking down stairs and left knee gave out and ee fell down three steps to stair landing.

Over-exertion (Not Elsewhere Classified)

Employee was walking from the mantrip to load center on a CM Section. EE stepped on uneven ground and felt a pain in the left knee. EE continued to work all scheduled shifts until EE was diagnosed with torn Meniscus. Lost time began on 4/3/2017.

Over-exertion in lifting objects

Employee was helping load roof straps on to Continuous Miner in the 4 west HG section and felt pop in shoulder. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies

Employee was operating walk thru bolter and twisted left knee while bolting in the #5 heading. Employee started missing work on 3/29/2017.

Struck by falling object

Steel liners fell from a set of springs and landed on employee's left foot.

Struck against a moving object

While operating Cat D9L bulldozer in reverse, machine hit rock causing employee's left elbow to bang against metal frame inside cab. On Tuesday March 28, 2017 employee was taken to local Urgent Care for observation.

Over-exertion in lifting objects

Employee told boss that they thought they strained their back but continued to work full shift and 8-5 hrs the following day. Told boss to turn in vacation day for the next day. Went to doctor that evening then missed the next day. Returned the following day and has worked since.

Struck by falling object

Employee was operating bore miner when the back peeled and a chunk of rock hit employee's foot and breaking it.

Fall from machine

Employee was stepping out of man bus to throw track switch when employee's leg got caught on man bus causing employee to trip and fall on employee's left arm resulting in tear to bicep tendon.

While scaling work area with 8' scaling bar, employee was injured while backing away from some loose EE scaled out of the back. EE tripped causing EE to fall. EE received a laceration to the left side of neck when EE landed on the ground requiring sutures. Also received a fracture to right foot when part of the loose EE had scaled contacted EE's foot during the event

Employee slipped on gravel straining lower back and injuring right wrist.

Over-exertion in lifting objects

While lifting a bag of rockdust, employee strained lower back.

Fall down stairs

Employee walking up stairway to work level to take down scaffold stumbled on sling ee was carrying. Lost balance causing ee to fall forward. As ee was going forward caught self with right arm to stop fall and felt pain in right shoulder. 3-24-17 Evaluated in medical first aid treatment. 4-10-17 F/U evaluation scan shows rotator cuff injury. Restricted duty.

Struck by falling object

After checking the load, the driver got a heel caught on rub rail and tripped. As driver started to fall they grabbed a cutting edge which fell off of the truck and onto their leg.

Struck by flying object

Cutting loose rock from roof with 12CM 15 miner in a high area in the St. Leo rehab/construction area and got the miner head too close to the rib. Caught rib bolt with miner head and threw bolt plate out, striking employee's left shoulder and causing a contusion.

EE was working on a 785 truck at Grape Vine South Surface removing the main pump with another EE and a field person from JP Tech. Pump was almost out, EE moved to the side and was waiting for pump to come out to the ground. When it did, it struck the pry bar EE had been using. When the pump fell to the ground, it hit the pry bar and the bar slammed into EE's lower leg.

Struck by falling object

Employee was replacing a panel on a shuttle car when a piece of draw rock approx. 23"" X 8' long and 2 1/2 to 5"" thick fell between bolts and slid down EE's back before striking EE's left foot. Foot wasn't broken but EE was hospitalized in order to receive antibiotics through an IV. EE was released on 03/26/2017. EE returned to work on 03/27/2017.

The injured's left leg became wedged between a battery coal Hauler and the coal rib on the #1 unit after the miner backed into the hauler and pushed it back into the rib. After transporting employee to the hospital it was discovered employee suffered a fractured leg.

Fall onto or against objects

Employee stated employee was attempting to dismount the operators compartment of the scoop. As employee was stepping out of the scoop employee's foot slipped, and employee's right shin struck against the frame of the scoop. This resulted in a contusion and lost work days.

Over-exertion in pulling or pushing objects

The employee was installing track rail on the 13 Right section. As employee pried a rail into place with a slate bar, they felt a sharp pain in the lower back. Employee completed the shift but called off on the next scheduled shift.

A roof bolt operator fell, striking head, resulting in a laceration.

An electrician was stepping into a tractor mounted basket platform and slipped in the mud, twisting knee. The ee was seen at the clinic, diagnosed with a sprain to the left knee, and put off work.

Employee was pulling cable off of the bolter and felt a pain in back.

Employee was trying to recover rail in the 1A spur. The rail was on top of another piece when EE tried to attach a chain the rail shifted striking EE's right hand causing three fractures. On the 24th EE worked a half day, EE left early due to pain & went to local ER. EE was seen & release to return to work on the 28th, EE was told to follow up with family doctor. EE saw family doctor on 28th & has EE off until further notice.

A scoop operator, was dragging a structure tray behind the scoop tramming inby. The injured ee was walking behind the scoop and actually walked into the tray, the tray caught on a piece of rail and flipped the tray in the air causing the ee to go airborne. The ee hit the mine floor and the tray came down on top of EE's lower torso.

Going through stopping located at 306-307 in the 9 south #2 entry and air pressure closed the door on EE's foot. When EE got foot out, EE fell down and stoved wrist on the floor trying to catch self, resulting in a possible fracture. No problems with mining methods, ppe, training or compliance issues.

Employee was riding in a scoop going to the section due to the super steer being down. The scoop hit a hole in the roadway causing employee's back to be jerked.

While re-positioning the drill the wire separated at adjoining seams allowing some small material (rock) in the basket to fall in front of the drill, startling the employee, causing the employee to retreat quickly and in doing so sprained left ankle in the process.

Employee was diagnosed with an torn rotator cuff and began missing work on 5/24/17 to have the right rotator cuff surgically repaired.

Employee stated they were hanging a reflector in the #1 entry on the #2 section. Employee stated they reached out to hang a reflector and felt a pop in the right side of the rib cage. This incident resulted in a lost time accident.

While building a crib, a co-worker slid another crib into place. The injured employees hand was caught between the two cribs resulting in a fracture to the tip of left middle finger. The employee missed two shifts then returned to full-duty.

End dump step bolts on R.S. fell out during operation. Employee was dismounting equipment using 3 points of contact. Employee fell backwards off the steps and landed on employee's back on the ground. MRI on 5/8/2017 revealed herniated disk and employee not allowed to return to work.

Mine was informed 6-2-2017 by temp agency work comp was accepting claim.

EE dropped a screw because it was cold out and fingers were cold. In looking for screw on ground, a gust of wind occurred and EE felt something hit EE in the right eye. It was determined EE had a corneal abrasion and conjunctivitis.

Injured was tail shear operator and while cutting out on the tail EE stooped under ring main hoses (around 252 shield, 3 East) and when EE raised back up EE hit EE's hard hat off the shield canopy causing pain in the neck area. Metal struck right arm and started to bleed. EE wrapped it up but it began to swell. EE went to Frankfort Regional Medical center after shift and they told EE to go to UK Sunday to get it removed.

Miner was dismounting equipment and stepped on errant, wind-blown debris causing a slip and fall with one foot on the ground. No first aid required, and work continued. On 7 March, miner complained of muscle spasm potentially related to prevoius injury, and was transported POV to ER for evaluation. Miner was advised to take it easy for a few days and prescribed muscle relaxer.

Employee was changing the belt on crusher screen when employee stepped down off conveyor onto ground standing on a material bridge of scraper cleanings. The material bridge broke loose and the employee slipped and fell, first catching self and getting caught on the roller and then falling hitting shoulder and elbow on the ground.

A fully trained miner was injured when releasing a breast line on a barge. The barge drifted and caught the employee's hand between the line and the barge. This was not a result of any compliance issues nor were any protective devices involved.

Employee was working on Sand Plant electric line project. The early morning snow had been plowed which created icy conditions. The employee walked around the corner of a 305.5 excavator when EE lost footing. When EE fell EE landed on the left knee taking all of the weight.

EE was descending a stairway in the B-regrind when right knee gave out. EE was able to put hands out to grab the handrails and to lower self on the stairs. EE sustained right knee sprain.

Employee and two other employees were sitting on the side of the scoop eating lunch. After the employees got finished eating lunch they got up and started walking so they could go back to work. They said they heard a pop and a rock fell. When the rock fell it struck the injured employee on the shoulder and the foot. The rock measured 36"" long X 12"" wide X 2 1/2"" thick.

Employee was assisting in reinstalling the top spring plate on a Deister screen. While doing so, employee's hand slipped off the plate as the plate was seated and got left pinky finger pinched between the plate gusset and screen gusset. Employee received 7 stitches to close the injury.

The IE was lowering a tow pin weighing 25 lb. on the bumper of a water truck. As the IE was trying to lower the pin, it got hung up and wouldn't move. As the IE applied more force, the 25 lb. pin fell 3"" to 4"" inches and struck the IE's hand, causing a fracture to the left thumb.

Interwest Supply employee (contractor) was working with a Barrick employee unloading 2x2 steel tubing from forklift onto steel rack hitting contractor on the forearm causing a cut. The contractor was seen at the ER and stitches were required to close the wound. EE was released to full duty effective 03/20/2017.

Employee was struck on their head by a tool as they were reaching for their hard hat which they had knocked off during their work activity. A laceration was received to their scalp requiring three sutures. The employee returned to work the same shift.

Employee was using a slate bar to pry on the axle of a diesel locomotive. The slate bar slipped, pinching employee's left pinky finger between the slate bar and a piece of angle iron.

While tailing-in a 10' PQ quill rod, the bottom of the quill rod kicked back causing the Helper to fall backwards onto a rod sloop resulting in a cut to the injured employee's right hip.

Over-exertion in lifting objects

Employee is a pro truck driver. On March 17, 2017, EE was lifting/pulling cable on the labor crew. After performing this repetitive work, EE noticed pain in EE's rt shoulder. EE did not have pain at rest. On April 4, 2017, employee went in for evaluation. EE was released without restrictions and referred to PT. EE recently informed the employer that EE began the PT sessions.

EE was arranging cover plates on stage loader conveyor using a come-a-long when hook came out of anchor and struck EE on chin creating a small laceration and breaking a front tooth.

The injured employee was moving a pickup truck from one site to another. After arriving at the site, EE exited the truck and shut the door. In the process of shutting the door EE's right second (index) finger was caught between the door and door frame. The resulting injuries to the finger were 4 sutures and a fracture to the distal phalanx (fingertip).

Employee tripped over pallets and fell. Sprained both ankles

While in the process of building a concrete brattice, EE was stacking the blocks and while doing so, the pile of blocks shifted and fell catching right middle finger between two (2) blocks. This smashed the tip of the finger. EE had to have 4 stitches to close the wound.

Employee was under a front end loader and reached to install a bolt in the starter. Dirt fell down into the eye of the employee. EE received a scratch to the cornea and a prescription was necessary.

Over-exertion in pulling or pushing objects

Employee was pulling a cord while working in sand. Employee caught their toe, twisted their right leg. which resulted in pressure being applied to the inner side of the right knee.

Slip and fall incident because of compacted snow conditions. Caused injury to right wrist.

Employee was removing a pin on the surge bin and slipped on ladder while hammering the pin and hit mouth with hammer damaging front tooth.

Employee was splitting a piece of pipe in two with a torch when the tip of employee's gloved finger got caught between the two pieces, pinching the end of the finger off.

Employee reported that EE put a split set bolt through wire mesh and lifted it over head and put the end of the split set into a drilled hole and as EE was attempting to maneuver the jackleg to drive the bolt, EE slipped and let go of the bolt and it came down and the bolt struck EE in upper lip causing a laceration.

Employee stated was lifting steel plate into place with chain hoist, tied the plate off to re-hook, released the chain hoist, and the chain slipped causing the chain hook to strike the employee on the cheek.

Employee was climbing down ladder in the whittaker shaft when ee stepped on a rung slipped on ice striking left knee. Employee has not missed work but is going to physical therapy. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills,proficiency,training or attitude. No PPE or protective devices on equipment involved.

EE had right hand on the drill pot on the roof bolter and while reaching to pick up a roof bolt with left hand a piece of draw rock fell striking EE just above the wrist. The draw rock measured 6"" x 8"" x 2"" thick. EE sustained a laceration requiring stitches.

While worker was repairing a water valve, a piece of draw rock fell striking EE on the right shoulder and head. EE continued working and began missing work on 3/27/2017 when taken off work by a physician.

On 3/14/2014, an employee took gloves off to wipe the sweat of their brow and continued to work. After an hour of work employee felt irritation to left eye and flushed eye for 15 minutes. Employee was seen at the clinic and no treatment was given. On 4/11/2017, the employee reported they continued to experience discomfort and was given antibiotics to prevent infection.

While cleaning on a CM conveyor motor, a piece of draw rock approximately 3"" in size fell hitting employee on the wrist resulting in a laceration requiring 6 sutures.

Employee was walking around rolls of conveyor at a belt drive when they twisted a knee in a rut in the bottom. The EE continues to work but started physical therapy on 03/24/2017.

Employee stated, employee was walking on the off side of the Belt. While walking the employee tripped over a crib block, and falling into belt chain, cutting over employee's left eye.

Contact with hot objects or substances

An employee was clearing a frozen slurry line using a steam hose. The frozen section released suddenly, creating back pressure, allowing the created hot water and slurry to spray out of the pipe opening and onto the employee's hands and arms.

Employee was carrying a chain over shoulder at secondary plant, ground was muddy, foot slipped causing the chain to swing around and hit employee in the mouth chipping a tooth.

Mobile Equipment mechanic cut left pinkie finger while removing metal shards from HiVac truck metal coupler. Shard cut through leather glove, requiring first aid and medical attention. Employee went to an emergency room for cleaning and 3 small repair sutures. No lost time, restrictions or medications.

Employee was walking to scale house when EE slipped and fell spraining ankle. Area was clean, dry, and level with no housekeeping issues. Employee is not sure if was about to climb stairs or had stepped up to first step when employee fell.

Absorption of radiations, caustics, toxic and noxious substances

Product bag burst with product going in to employee's eye, behind safety glasses.

Over-exertion in lifting objects

EE was helping another laborer move a negative air machine from one vessel to another. The EE aggravated a pinched nerve from a previous injury. The EE requested to be laid-off. On 3/21/2017, we were made aware that EE went to a chiropractor for treatment.
