An extension of a previous non-injury rock fall was discovered in the intake on #11 belt break #215 of the primary escape way (wall entry). It is approx. 12' thick 30' long and 18' wide. The affected area has been cribbed and dangered off from all points of entry. Mine has been idle since April 2016.

The elevator went down at 4:45 PM. The elevator brake coil over heated or was weak. The brake coil was changed and all safety inspections and safety functions were conducted before the elevator was returned to service.
May 27th 3:11PM D11 426 caught fire. The miner was pushing and notice smoke come from front for the dozer while backing, ee stopped to investigate ee smelled smoke and found fire below the dozer on the Rt. side near the cab. Fire suppression and fire extinguishers were unable to extinguish the fire. Fire Dept used a foaming unit to extinguish the fire No injuries.
An unplanned roof fall was discovered at #5 block on the East Mains section, in #2 return entry. There were no injuries, and no equipment damage, and the ventilation was not disrupted. This event was immediately reported.
At approximately 11:20 pm the pre-heat tower elevator with four miners inside stopped between floors 3 & 2. Miners were in constant communication with outside personnel. At approximately 2:16 am the elevator repair person arrived on site and the miners were removed at 3:17 am.
During backfill process at the 1675 level, operator noticed material movement coming from the back over the open stope.
The #9 Portal elevator was taken out of service at 12:30 am when it was showing errors. No one was on the elevator. It was put back into service at 1:10 am after the governor switch was adjusted.
A roof fall was discovered in the intersection of Main West #4, crosscut #27. Fall was approximately 23.5' x 23' and 11 feet high. Exact time of roof fall is unknown.
A roof fall has occurred at #16 Break, #3 Entry, 9M CM Unit
A roof fall occurred in the Left Return on the #5 Belt line inby SS# 832 in the #1 Entry and SS# 831 in the #2 Entry.
The mine experienced heavy rain storms over the weekend and the water entered the mine opening from the new pit that is under construction. When the mine was examined Sunday, the water was already out of the mine.
The elevator went down at 5:00 PM. There was one employee on the elevator when it went down, the employee was conducting the elevator inspection during this time. The rope brake was locking up, to correct the problem we changed the e-stop switch on top of cage.
Roof fall was discovered in the belt entry between break 28 and 29 on # 2 South belt. All approaches to the fall is breakered off.
A continuation of a roof fall was discovered adjacent to the #2 light side track.
A low wall in the first box pit fractured letting overburden material sluff into the pit
A wheel on the weight car attached to the slope car broke off. Production was stopped and all miners withdrawn to outby the feeder. The primary escapeway is still available and the alternate escapeway is still travelable. The weight car was removed from the slope car and the slope car was available at 4:12pm. No persons were injured or killed or trapped during this incident.
At approximately 13:00hrs a non-injury roof fall occurred on the #2 section - Castle Branch No.1 Panel 007 active MMU.
The slope car used for hoisting men in/out of the mine had an electrical malfunction. A production was stopped and miners withdrawn to outby the feeder. The primary escapeway is still available and the alternate is still travelable. No persons were injured or killed. The slope car was repaired at 10:55am
A non-injury roof fall, approximately 30' long and 20' wide occurred in the #7 entry at survey spad #11536.
An unplanned roof fall occurred in the 1st Left Panel travelway off the 4th SE Main. The roof fall occurred at crosscut # 13, entry # 5. The fall was approximately 18 feet wide x 20 feet long x 8' thick. The area had been supported with 6' fully grouted roof bolts and supplemental 8' fully grouted roof bolts with straps in the intersection.
At approximately 11:30 p.m on 5-14-17 the section boss and safety person heard what sounded like a roof fall. As they started looking across the faces they observed a roof fall in the #5 entry. The roof fall was dangered off and no one was injured. Both agencies were notified.
A crack in the spoil area was noticed and the mining operation was pulled out of the pit. Later on the spoil slide into the pit but all employees were out of the area before this happened. MSHA was notified and inspector came out to the site. Mitigation plan was put in place. No injuries.
A roof fall occurred in 67 Headgate section at 47 xc #1 entry. The fall was at an intersection and was approximately 16' long 20' wide and 18' high. No one was injured or entrapped. The fall occurred due to a failure of the over lying strata above the anchorage point of the roof support.
An unplanned roof fall was discovered at 6:50 am in the 16 East Bleeder. The fall is in the #1 Entry at the #2 crosscut. The fall is approximately 20' high, 15' wide and 80' long. The area has been dangered off and bolting is in process.
Unplanned roof fall at XC #42, entry #2, station #6218, 4-L Headgate. Fall occurred in an intersection, 4-way, supported with 8' TR bolts and 12 ft. cable bolts (star pattern). Fall dimensions - 25 ft. long, ,19 ft. wide and 12 ft. high. Did not cause injury, inpair ventilation or intrap miners. Fall area developed 5/4/2017 to 5/7/2017. Reported to MSHA at 7:20 am 5/10/2017.
A highwall slough occurred in the C Area Pit. The section of highwall that sloughed was approximately 300 feet wide and material went about halfway across the pit floor. No miners were working in the immediate area, but miners who were working in the pit, north of the slough were withdrawn.
A roof fall was discovered in the 17th Right Section, entry #3, outby crosscut #5. Approximate dimensions of fall are: 18.5 feet long, 7.7 feet high and 15 feet wide.
This area was first found on Monday 5/8/2017 and at that time it was just a pot out. All 4 approaches were dangered off and floor to roof support was installed. The next day 5/9/2017 is when the pot out had fell out more and was then above the anchorage point.
An unintentional roof fall occurred in F27 Longwall section between 26 wall and 25.5 wall at spad numbers 63+60 to 63+88 in the #1 entry. The roof strata separated above the anchorage and fell to the ground. The fall measured approx. 10ft high by 28ft long by 12ft wide. There was no injuries as a result of this incident.
Mine elevator became stuck with workers on the elevator; the elevator first stopped at 7:30am, with maintenance personnel working on the elevator, getting it to move a few times. At 8:30am the elevator was deemed stuck and MSHA was notified.
The #9 Portal elevator went down at 2:45 am with electrical control issues. A new mother board was installed and the elevator was back in service at 11:44 am of the same day.
A roof fall has occurred in the #1 entry at spad #409 in the main return. The fall measured 20' wide x 20"" long x 6"" thick. This area will not be rehabilitated at this time.
A fall has taken place above anchorage in the #2 entry at survey station #12442 of the Castle Branch #1 Panel. The fall measures approximately 10' x 14' x 8' high.
A Mine Examiner discovered a roof fall in the #1 intake entry of 7 North Mains just outby 31 wall. The fall originated just inby a permanent ventilation stopping located in the entry and traveled approximately 19 1/2 long and 7 1/2 feet wide. There were no intersections affected by the fall. No persons were injured or equipment damaged as a result of the fall.
On 5-2-17 at approximately 1:51 a.m. the service hoist went down due to power issues. No one was trapped or injured. Power was reestablished and the hoist is back up and running.
The production skips were being adjusted in manual mode. The DHS system showed that the skips had tripped a safety. A switch was depressed by the hoist operator causing the DHS to prevent the skips from moving. All personnel were evacuated from underground while the system was reset and troubleshooting took place. The production hoist was back in service 8:20pm
A roof fall occurred in the 67 Head gate section in #2 entry at 47 crosscut. The fall was 18' to 20' high and extended the width of the entry plus 30' into the 2 to 1 crosscut and 30' into the 2 to 3 crosscut for a total length of 80'. No one was injured or entrapped. **Roof fall occurred due to a failure in the overlying strata.**
EMPLOYEE WAS OPERATING #43 DC MOTOR ALONG 8 HAULAGE EXTENSION TRACK WHEN EMPLOYEE STRUCK EMPLOYEE'S HEAD AGAINST A ROOF SUPPORT BEAM. INCIDENT INVESTIGATION ONGOING
On May 6, 2017 at approximately 16:55 a haul truck operator backed up to a berm to dump, the berm breeched and the haul truck and berm slid down the highwall approximately 150 ft. There were no witness to the incident.
Miner was tramming roof bolting machine out of the face of the No. 2 entry from the operators deck and smashed pinky finger between the coal rib and the frame of the machine. Resulted in lacerations and fractures of the pinky finger.
Employee went to stand up and turn and twisted employee's left knee.
Employee was greasing tracks on highwall miner when ee fell and twisted right ankle
EE was performing a pre-shift inspection on a Cat 988 front end loader, was attempting open a stuck access door to the engine, when the door was freed it came off of its hinge causing the employee to lose their balance and fall to the ground injuring a rib.
Ee was filling an accelerator tank when some accelerant splashed up into the ee's left eye. Ee was seen at the ER and diagnosed with an alkaline burn to the eye. Subsequent follow-up with eye doctor continued lost time and prescription meds for the eye.
Shift started at 6pm prior evening. During the working shift the employee was clearing raw kiln feed from a transport drag by vacuum method. The hot feed was being cooled by water application to prevent vacuum hose from collapsing. The hot material sloughed into pooled water creating a steam rush that burnt the miners legs. Incident investigated by mine inspector.
The employee was scaling the mine roof. A piece of rock fell on employee's head and back, driving them to the ground.
Employee stated while riding a Mac8 to the section, the driver backed into and dislodged a timber which fell striking ee on the hardhat. EE did not loose consciousness, refused backboard and was transported to the surface by EMT with a cervical collar in place. EE was transported to hospital via ambulance and diagnosed with a concussion and given one week off per protocol.
While dragging crib in 4 West airway, employee felt pain in lower back. This did not involve any compliance issues. Personal protective equipment was being used, and this did not involve job proficiency, attitude, or machinery.
EE was servicing a lube truck and while walking around the truck ee stepped on a lump of coal which caused to fall to same level resulting in pain to left hip/leg.
Employee was pulling timber into position with a pick hammer and strained themselves. Has swelling in the groin area. Employee started missing work on 6/1/2017.
The employee was throwing the loader cable to the rib out of the roadway when employee felt pain in employee's right arm between the bicep and forearm. The employee worked till June 4, 2017 when employee went to the Dr. and was taken off work for a arm strain.
Conditions were good. EE was climbing water pull truck using 3 point contact. EE placed foot on Top step and slipped off losing balance. EE lost footing and grip and fell to the ground with most of the weight focused on the left foot. EE heard it crack. EE called safety manager and I took EE to the hospital. Result is a broken L-ankle .
Employee was walking in the belt entry and slipped/tripped/and fell, injuring right hand index finger.
EE was washing the chute and had left arm over the chute when the platform EE was standing on broke and EE's left arm got caught on the top of the chute.
Employee was hauling overburden from top of pit to overburden area in an A25C Volvo articulate. As employee neared the bottom of ramp employee turned vehicle to left causing wheels to climb berm which caused the vehicle to turn over onto its side. Employee hurt right shoulder and had a laceration on left shin.
While re-railing a supply car employee felt a burning sensation in abdomen while using a track jack. Employee continued to work until the doctor took EE off work starting 7/24/2017. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, machinery, or attitude.
Team member was climbing the ladder on the locomotive. Team member reported that EE heard popping noise in the right wrist. Poor body mechanics.
After digging out a plugged tail pulley the employee went to start the conveyor belt when a rock 4""X6"" fell from the belt striking employee's left hand between the thumb and index finger.
Employee was palletizing rocks when ee pulled a stone from pile and another shifted and smashed middle finger on right hand.
While shoveling natural sand at the dozer trap, worker stood up and heard a pop in the knee.
Employee was climbing down from pay loader when they slipped and lost their step causing their right hand to hit hard on rail above. Employee worked 5 days thinking they had only bruised the hand. On the 31st they went to doctor and found that they had fractured the 5th metacarpal in the right hand.
Slipped on dragline hurt back. But didn't report it until Tuesday May 30th 2017.
Employee was walking down entry of section, as employee stepped over miner conveyor chain and rock, their foot slipped causing them to fall back on their foot resulting in fracture to lower leg.
Employee was kneeling down and was cribbing under the continuous miner when a rock fell, hitting EE in the top of head and slid down EE's right shoulder.
Employee was stepping down from the step on the left side of the miner and stepped on a piece of coal and twisted right knee. Diagnosed with a possible medial meniscus tear on 6/6/17.
Operator of Brookville locomotive violated movement instructions thus hitting mantrip head on. Employee was hospitalized 3 days with back injuries. X-cut 83.
Employee was attempting to chuck a drill steel into the jackleg, when the crow's foot of the leg kicked out causing the jackleg to fall. In the attempt to keep the machine from falling to the ground, employee fell pinching right thumb between the machine's handle and a wood board. Seen by site medic, referred to ER, X-rays positive, sutures, and restrictions issued.
While side bolting, employee was raising a roof strap to the roof when a piece of rock 20""x14""x6"" fell out from the roof and hit employee on employee's right knee. When employee fell, employee caught self and jammed employee's left wrist. Diagnosed with a blood clot in employee's right leg on 6/1/17.
Operator of Brookville locomotive violated movement instructions thus hitting mantrip head on. X-cut 83. Employee treated and released at CAMC Emergency Room with cracked vertebrae. Referred to specialist.
Employee was removing a 3"" dust hose off the back of the rock duster when the hose came around and hit the employee in the right upper arm area.
Employee was walking past the tail end of 13 belt when ee saw the sample port door was open. EE stepped up on the platform to close the door. After the door was closed ee stepped off the platform and rolled left ankle on a large piece of clinker. Ankle had a large amount of swelling. On 5/31, EE went to doctor who ordered ee out of work in order to allow healing.
Employee stated that employee strained employee's right shoulder when employee's foot slipped from the step as employee was dismounting Cat excavator. Employee did not begin missing work until 6/1/2017
EE was installing some bolts on a pipe flange. As EE repositioned self, EE felt a pop in the right knee with instant pain. EE was taken to the ER. EE was taken off work and referred to orthopedic doctor.
Employee was getting off of the dozer and slipped, striking employee's left shin on the push arm of the dozer. Employee continued working until 5-31-2017.
Employee was blowing duct work down through 1 inch port, dust blew out of port and under face shield contacting skin on face causing alkali/lime reactive burns to face.
While working as a pumper installing cans in the longwall entry, the injured employee turned to hand a pogo stick to someone and felt a pop in knee. EE was transported by ambulance to the hospital with a sprained knee and has not returned to work.
Employee stated that EE was tightening some bolts on the shaker system. While tightening the bolts, the wrench slipped and EE hit right elbow on a piece of the housing on the shaker system. Employee did not start missing work until 6/26/2017.
Employee was putting rigging into mantle while lining up eye bolt, crane hook drifted down and smashed left thumb, nail was removed and 3 stitches applied.
Employee was exiting personal vehicle prior to the beginning of shift and EE slipped on ice in the east parking lot. EE strained right shoulder.
Employee and co-worker picked up a bottle of nitrogen to carry to the truck they were working on. When employee felt a burning sensation in left shoulder.
EMPLOYEE WAS PUTTING LAGGING OVERHEAD WITH THEIR PARTNER. IT WAS A 3X12X6 TIMBER. THE EMPLOYEE LOST HOLD OF THEIR END AND IT HIT THEM ON TOP OF THEIR HARD HAT.
Plant operator was cleaning out around overloaded screen unit. While doing the clean out a rock fell on their finger. Plant supt. transported to Bluefield Regional Medical, treated and released. No stitches or sutures.
Employee was removing rock dust bags from a supply train when EE felt a pain in lower back and shooting pain down right leg. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, machinery, or attitude.
Tripped getting out of skid steer, and leg got wedged between bucket and skid steer frame.
While lining up a drive shaft on a shuttle car, the drive shaft slipped off of the yoke striking employee on thumb area of right hand. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, machinery, or attitude.
An employee was replacing liners when the platform shifted causing ee to lose balance. EE jumped up and landed on heel causing a fracture to the heel.
Stepping out of a diesel locomotive employee's left knee failed. Employee didn't violate any laws, rules or policies, has proper training and has a positive attitude for safety. Employee didn't have a specific incident and is believed injury is a result of a body failure type injury. Employee was wearing all required PPE.
Employee was hanging curtain and a piece of rock fell between a bolt and the rib and hit employee's foot.
Descending ladder of 4600, foot slipped & employee grabbed ladder causing pain in shoulder.
Low coal seam 38"" and area is dry. Lifting a part for the miner employee felt a strain in employee's left shoulder.
An employee was standing on a Ramcar, washing it off, when another employee started to move a nearby Ramcar in the opposite direction. The moving Ramcar caught the roof bolter cable hanging from bullhorns, pulling the cable down. The moving cable forcibly struck the employee standing on the Ramcar, knocking that employee down on the Ramcar.
Contract employee was taking a mop bucket out to the janitor closet outside of the dry. EE slipped backward on the wet floor and fell, and was struck in the face with the bucket. EE was transported by light vehicle to NNRH with a cut over the eye. Subsequent follow-up at University of Utah resulted in stitches and 3 days off.
Employee was trouble shooting an electrical transformer using a Fluke multi-meter when an electrical arc occurred resulting in second degree electrical burns to EE's right arm.
While riding out on Damascus ride #352, employee had their foot on top of the fire extinguisher guard and as the ride went through the dip on 11 belt, 12 1/2 block, the employee's left foot struck a roof bolt in the top causing leg to be bent back towards the employee. The preliminary results are at this time possible ligament damage. We are currently waiting on MRI results.
Injured was fire bossing in the 6 mains area of the 4-west mine when they slipped in mud on a wooden walkway and strained right knee. Injured continues doing regular job duties until 9/22/17 they began missing work due to knee issues and under Dr. care.
Employee was attempting to set power on No. 2 circuit breaker when the power center went phase to phase causing burns to employees left hand, arm and neck.
Employing was hydro-blast under Precipitator #6. Positioning platform shifted due to back-thrust on blasting shotgun. Employee lost footing and struck elbow on guardrail.
Employee stated ee was operating a shuttle car in the #8 entry and the shuttle car got close to the line curtain. In order to prevent tearing the curtain down the employee jerked leg over to hit the brake. When ee did this ee felt pain in groin area. This resulted in a strain and lost work days.
A fully trained miner injured left ankle when ee was unloading belt structure. The employee stepped on the edge of the scoop bucket and foot slid causing ankle to roll. This was not a result of any compliance issues nor were any protective guards involved.
Employee was assisting the loading of a belt tub onto a well bottom car. The tub needed re-positioned. Employee motioned the scoop operator to ""slew"" the tub over. The operator shoved the tub. Employee's foot was caught between the anchor point of the car and the tub. First aid was administered. Employee was transported via ambulance to a local hospital.
While closing mandoor, employee's right middle finger was caught between the door and door frame, pinching, resulting in sutures.
Employee stated that as employee was walking to the #2 entry face in the last line of breaks employee stopped in the intersection at SS# 1400 to pick up a pop can. At this time a section of the inby rib sloughed off striking employee's upper back.
Looping high line cable in sled and while pushing down on high line cable felt a pop in right shoulder. Employee started missing work 09/06/2017 due to surgery.
Employee bent over to hang a pull rope on the continuous miner and felt a pain in employee's back when employee stood back up. Started missing work 5/19/2017.
Employee was unloading a tub of assorted belt structure. As ee reached for a piece of structure, a belt rail slipped down from the stack and pinched the tip of right index finger.
Employee was operating a shuttle car on the #2 section when the car bottomed out and became hung in the intersection. When the shuttle car was freed from the hole in the mine bottom, the employee was jarred inside the operators compartment. This resulted in a neck/back strain and lost work days.
Employee alleges that while driving into work on the plant entrance road the vehicle they were driving was struck from behind while sitting stationary causing injuries that required ambulance transport to the hospital. Reason for late filing of 7000-1, Mine Operator was unaware plant entrance road (location of incident)was in the mine jurisdiction.
Employee working on #2 unit assisting miner operator on left side of section, lifting rub rail on miner heard and felt a pop in left bicep.
Employee was taking rock off of a conveyor belt to place in a metal box. While grabbing one rock another rock came off at the same time landing on employee's left index finger.
Pulling the cord to start the pony engine for the crusher and the cord broke causing ee to slam elbow into the step rail causing a cut. EE went to health reach and received stitches. Employee elected to take a sick day the next day.
Employee was using a cutting torch to cut a steel beam, approx. 200 lbs., 5"" wide, about 1 foot from the ground. As ee was cutting the beam, it shifted and fell across both feet. EE right 2nd toe was fractured and ee sustained contusions to both feet.
Employee was placing floor grating on a cobber platform. While moving it to its final position, it slightly snagged so employee had to lift upward with the load. EE felt left ring finger pop. On May 30,2017, ee was taken off work.
EE was attempting to move a 2 inch air pump from 59 cross-cut on the belt line to the sump.EE thought the air line and depressurized and attempted to move out from the belt area. When moving the pump, the air line came off of the pump, which caused EE to lose balance and fall into the old structure and rollers, causing a sprain to right wrist and right knee.
Employee was struck in the back by loose material and driven down into a pile of spillage.
The rigging came off the liner handler and the grate hit the outside of employee's foot, bruising it.
While re-railing a mine car, the hydraulic jack kicked out and struck EE on the right arm/hand, causing a fracture
As the employee stepped down from employee's bull dozer, employee felt a sharp pain in employee's right knee. Employee had difficulty straightening the knee. Employee completed employee's shift but called off on employee's next scheduled shift.
Employee was walking down the slope belt when a small rock came off the belt hitting the employee in the left ankle.
A nail used to secure the free end of a belting roll was allowed to remain in the belt and was pointing up as the free end was lying on the ground. The employee stepped on the nail.
EE squatted down when raised up & turned felt pain in lower back, went on to section started hanging curtain. EE said pain got worse, went to local ER. EE stated the dr. said it was muscle spasm. EE went to family dr. next day, EE is off work do not have a return to work date.
Employee while greasing loader tripped on grease house and strained employee's right knee
While working on Simons Crusher the miner was loosening the bolts to the crusher spring pack when the bottle jack supporting the spring pack bottom plate displaced out towards the miner from the bottom of the spring pack plate striking the minor on the left forearm.
Employee was setting up a breaker on a 995 volt miner when the cst head blew out burning both hands of the employee. Both hands received second degree burns. Employee was off from work for 20 days and returned to work on 6/14/17.
EE was attempting to take down a mono-rail beam on the tight side of the stage loader. As EE reached up to take the chain off, EE felt a pain in the lower left back.
Employee was operating a scoop and ran over a rock. This caused the employee to be jarred inside the operating deck of the scoop.
While walking by a portal bus, the employee caught the safety hook of the bus with EE's left foot. EE fell and twisted EE's ankle. EE finished the shift and went to EE's doctor later in the day due to ankle pain. X rays revealed a fractured ankle. Employee was scheduled to see a specialist on 5/18/17 and has not returned to work as of today.
Employee was prying on a screen to remove, felt a burning in lower back.
EE had just loaded dump truck w/gravel using loader. It is believed that EE passed out, due to a high fever, while backing up. The loader backed into the berm and tipped over on its side. Truck driver helped EE out the loader. Another EE took EE to the doctor because EE was disorientated and trying to go to sleep. The accident resulted in EE having a concussion.
Miner was dragging a post through the walkway to the #6 seal in the Main North Junction seal set when it got stuck. EE jerked on the post to free it and felt pain in right shoulder. MRI showed a full-thickness tear of the supraspinatus tendon and shoulder joint effusion. No problems with mining methods, ppe, training or compliance issues.
Employee states while getting out of the battery hauler on #1 unit, Employee hit the breaker from inside the cab and while getting out, the cab section of battery hauler rolled on EE's right heel.
While using a pipe wrench to move a bolt on a conveyor belt, the wrench slipped and employee's hand struck the floor causing pain in left wrist.
SIMBA OPERATOR WAS TYING A RAG ON HYDRAULIC HOSE TO MARK IT WITH MACHINE ON AND GOT A OIL INJECTION PIN HOLE CUT TO RIGHT HAND PINKY FINGER.
While working on the suction system on the angle bolter, employee pinched right middle finger with the dust box door. The laceration required 5 stitches to close.
The employee stated was dismounting a D-7 Dozer, and slipped off the push arm of the Dozer. The employee stated as the employee fell the employee landed on right shoulder. Employee sought medical attention and was diagnosed with a fractured shoulder. Injury being reported as Lost Time Injury.
Employee was placing base lift back on the shield feet, when EE felt a strain in the neck and back. Employee did not start missing work until 5/18/2017.
EE was dismounting the 4600 Drag Line. When EE stepped on the second step from the top EE's foot slipped off. This caused EE to loose EE's handhold and fall to the ground. This resulted in a fracture to the LEFT hip. EE fell approximately 4'.
The employee was using a wash hose at the Primary wet screen. Employee was coiling the hose and dropped the nozzle on the concrete causing the water to come on. As employee tried to recover the hose, they twisted awkwardly pulling a muscle in their back.
Employee was walking in F28 TG alongside the exhaust fan tubing. The tubing stretched from the straight of the #2 entry around the corner of the 2-3 track chute to the return. The slack in the tubing shifted quickly and knocked employee to the ground. Employee fell on their right ribs. Diagnosed with two broken ribs.
Employee stated that while making up bolts just outby the drill station of the bolt machine, preparing to install the 3rd row of bolts in a cut, a section of top fell out striking EE on top of EE's head/upper back.
Pulled muscle in shoulder while performing plant maintenance. After medical review, suggested that EE relax for a day or two.
A fully trained miner was cutting conveyor belt when the knife came off of the belt and cut into EE's Rt. leg. No compliance issues were a factor nor were any protective guards involved.
Climbing down (dismounting) a Caterpillar 930H end loader and stepped on a piece of metal on the ground, rolling an ankle and falling down into a wall, resulting in an ankle sprain. No problems with mining machinery, PPE, training, or compliance issues.
EE was removing a top roller segment from a damaged belt rail. As EE removed the roller the belt rail shifted and struck EE on the front of the right thigh. EE received abrasions and a large contusion to the right thigh.
On 5/11/17 employee cleaning tank with high pressure washer. Water stream from high pressure washer contacted their foot causing a laceration.
Employee stated they were walking in the #3 entry when their foot got stuck in the mud causing them to lose balance and fall striking their right shoulder on the mine floor.
Tire technician was in the process of dismounting a 33.00X51 tire on the LF Steer of an end dump rock truck. When technician released the lock ring, it fell and rolled around and struck the technician's right big toe.
Employee was carrying mandoor when they stepped in a hole and injured knee.
Employee was packing down shim box and aligning tools on incline and felt right knee twist.
Co-worker and IE were removing a bolt and nut to remove a troghlin idler. IE was holding a combination wrench to hold the nut from moving and the combination wrench slipped and pinched IE's right index finger against the angle iron and wrench.
Injured was walking on the belt line, mining height approximately 52"", and struck their head against a support beam. Went to the doctor on May 15, 2017 and was diagnosed with strain/sprain to neck. Injured is restricted to no work until follow up appointment.
Employee repairing a cable for slushier, had to cut a section of cable, using the flat side of a mine axe & a cable cutter. With the axe in right hand & holding the cable cutter with left, the axe head glanced off cable cutter head contacting left thumb. Seen by site medic, referred to ER, X-Rays positive for fracture, restrictions issued.
Crew was pulling a water supply line using a rope hanger. Water line pulled tight causing the rope hanger to break striking employee in the face just below the right eye. Resulted in a laceration and fracture.
Worker attempted to loosen a bolt with a wrench, hand slipped off causing employee to fall from their feet.
At approximately 8:30am an accident occurred at the Bailey prep plant course refuse area. While an employee was walking to the dozer EE was struck by a Caterpillar road grader that was traveling in reverse.
Employee was lifting a flume screen for replacement and felt a sharp pain in employee's lower back, employee continued to work employee's regular shift until 06/26/2017, plant was notified on 06/29/2017 that employee's Dr. had placed employee off work
Skid load rolled pinning feet against rock pile.
Employee had done work on the 216 Cat skid steer and had started letting the hinged canopy down when the employee feet slipped causing the front of the hinged canopy to hit the employee in the top of the head. The employee fell backward landing on employee's back between the bucket area and the frame of the skid steer.
Employee was delivering shields to the 2 East Longwall set-up by tram. When employee exited tram employee stepped on a pile of gravels and twisted employee's right ankle resulting in a sprain/strain injury
EE was a passenger in a diesel mantrip. When the mantrip picked a switch the EE grabbed the hand rail inside the mantrip to brace self. EE felt a pop in right shoulder. The EE has had two previous surgeries to this same shoulder.
EE stepped off a diesel jeep into a hole and twisted EE's left ankle and fell to the ground.
Employee was troubleshooting the miner. The power was unlocked and set up to check the relays for functionality and an arc flash from the panel burnt the employee's forearm.
IE (Temp. employee) was roof bolting on the right side of Unit 2 in Room 2 at 1+20. EE was installing a roof bolt and reached up to rotate the roof bolt plate when EE got right index finger caught between the plate and the mine roof. EE received a facture to the tip of the finger and (3) sutures to close the laceration.
Employee was cleaning iron oxide (umber) from inside the bottom cone of the bag house; inserted right hand into enclosed chute leading to moving air lock; fingertip of right index finger was partially amputated.
Using a wash down hose to clean the belly pan on 16 belt (prep plant, plant feed belt). The hose got stuck and when employee went to pull on the hose employee felt a pain in employee's lower back. Employee has history of lower back problems. No problem with mining machines, training, PPE, or compliance issues.
Abrasion to cornea of right eye from foreign body falling behind safety glasses while walking from Meal Bin Tunnel to Meal Bin Office
While dragging post's into 4 West occasionally post would get caught on rail. Employee finished shift with no indication of injury. After going home employee said EE woke up the next day with pain in lower back and hip area. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.
Dumping load - Malfunction on hydraulic system - Box came down - rocking truck causing injury to employee's back.
Ee was coming down off a scissor deck, put EE's right foot on the ground, went to put other foot down and EE's right foot rolled off the tire trench EE was standing on. EE felt immediate pain, was taken to the clinic, diagnosed with an ankle sprain and released to modified duty, non-weight bearing and required to wear a medical boot.
EE was building a stopping in a X-cut. When they lifted a concrete block up to stack it, they felt a pain in the left shoulder. EE missed the first day of work on 07/27/2017 due to the injury that happened on 05/09/2017. This report had already been submitted on 07/31/2017 but hasnt shown up in MSHA's system so I am resubmitting.
Employee was using a manual hand pumped grease gun to grease rollers on a conveyor belt line when they injured the right arm and shoulder. They took themselves to the hospital and missed no work until today when they brought in a doctors excuse stating that they will be off from work for 16 days.
Employee was setting bolt and spinned the torque tension bolt backwards and snapped head off the bolt. The plate flew off and hit employee in the forehead above the left eye
Employee was retracting the evac hose after use on 25 grader. It started going too fast so EE grabbed it to slow it down. The action caused pain in EE's right shoulder. On 6/16/17, EE had surgery on the shoulder.
EE was in the process of knocking a hole in a stopping to push props through. As EE took a swing at the stopping with the sledge hammer the head of the hammer went through the hole causing EE to smash left ring finger between the block wall and the handle of the sledge hammer. This resulted in a fracture to the tip of left ring finger.
Employee was laying a block for the foundation of a stopping. As employee turned to pick up another block employee heard employee's left knee crack.
Employee said got out of truck to turn heater values off then climbed up the step to the side of engine as employee stepped back down felt a sharp pain in left hip.
The injured miner was assisting an electrician in checking extension cables leading to a stacker. The power circuit being tested was not de-energized before the work was done. When a plug-in was connected the injured miner holding a test wire received an electrical shock and arc flash burns.
Employee stepped off the stairway onto the ground. When ee stepped with left foot, it slipped in the mud and ee felt a ""pop"" in knee. EE continued to work until 5/25/2017 when ee was diagnosed with a torn meniscus.
Employee stated that as employee was tramming back toward the section feeder the shuttle car slid toward the rib where there was a crib block. The crib block slid into the shuttle car and struck the employee on employee's left lower abdomen.
A fully trained miner was cutting a conveyor belt when knife slipped from the belt and cut Rt. leg resulting in a laceration. The injury was not a result of any compliance issues nor were any protective guards involved.
Employee was struck in the back by a rock measuring 2' wide x 2' Long x 1 1/2"" thick, causing a contusion to the employee's lower right back and hip.
Employee was standing on bull dozer track attempting to put antifreeze in dozer when employee's foot slipped causing employee to land on their side on the lift jack of dozer causing pain to ribs and causing chest wall contusion.
Employee was pulling back on the jackleg machine after finishing drilling a hole in hanging wall and the drill steel hung causing machine to torque. Employee lost balance from machine twisting and fell straining lower back. Seen by site medic & referred to ER, X-Rays showed negative findings, restrictions issued until seen at next level of care.
A contractor employee was unloading high voltage guarding from a man trip. As employee was walking backwards stepped into a hole causing employee to sprain left ankle. Employee continued to work until 5/15/2017.
Employee fell while traveling through a man door and the door closed on leg. As employee reached back to attempt to free self employee felt a pain in right knee and lower back. Became a lost time on 5/8/2017
While lifting a 12 volt battery out of a golf cart, EE felt a burning sensation in the area of the navel and above. Surgery was performed to repair a hernia on July 5, 2017 during vacation shut down. EE started missing work on July 10th when the mine resumed operation.
Employee states that ee had bent over and was hooking up a shot lead line and when ee straightened up ee felt a sharp pain and a popping noise in lower back. Employee was examined by physician on May 9, 2017 and was taken off of work.
Employee was installing a roof bolt in the #1 heading on 3 East Development when a rock fell and struck injured in the right hand resulting in a laceration on the middle finger and a laceration on the top and bottom of the ring finger. This also resulted in a fracture to the proximal interphalangeal joint on the right hand
Employee strained back while attempting to hang miner cable.
A fully trained miner injured right leg when EE was trying to pull structure from under a supply car. When freed, the structure contacted EE's right leg. It was not a result of any compliance issues nor were any protective guards involved.
Scraper Operator was operating scraper and didn't report an exhaust leak. Exhaust leak was fixed as soon as mechanic was aware of the problem and the machine was back in service before the end of the day. Employee went to the hospital at the end of shift.
Employee put both hands on the lip of a trash car when a roller that was in the car rolled and struck their pinky finger between the roller and the side of the car.
Clear and sunny. Lost footing on ladder entering Loader, resulting in a fall. Fall resulted in a right shoulder injury.
OP3 was in area 335 turning a valve on #8 press when they felt a pain in the right abdomen that radiated down into groin. Employee seen by doctor for evaluation.
Driver was attempting to dump a load of BASF material. The load would not come out so the driver was lowering bed when the load shifted and the truck and trailer turned over.
While using a hand crank to raise the drop door on a ballast car with EE's right arm, the door got stuck. EE went to apply greater force and felt a pain in EE's posterior right forearm, 1"" distal of elbow. Referred to specialist for evaluation, diagnosis and treatment. No problems with mining methods, PPE, training or compliance issues.
Employee was loading, unloading and setting 6X6 timbers when employee had pain in employee's left shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF INJURY UNTIL 6/23/17 WHEN EMPLOYEE WENT OFF WORK FOR ROTATOR CUFF SURGERY.
Employee was attempting to remove a pin from the shear jack of the continuous miner. Pin became lodged, employee attempted to use slate bar to pry the pin away from the frame when the bar slipped causing a strain to lower back.
EE stated they slipped getting off D8 CAT dozer at the gravel pit and twisted a knee. No witnesses to this accident, and EE didn't report this to a supervisor at any time. I was notified of the incident when EE filed first report for workman's comp on 5/12/17 with HR. People did see EE limping and asked about it, but EE didn't report this to anyone. Date & time are in question.
While moving a super post to the top of a fall, post slid down unexpectedly crushing the left ring finger between the post and a rock. This did not involve any compliance issues. Personal Protective equipment was being used and this did not involve job proficiency, attitude, or machinery.
WORKER WAS STEPPING BACKWARDS WHEN EE CAUGHT THE EDGE OF A SKID WITH SHOE. EE LOST BALANCE AND FELL BACKWARDS ONTO THE CONCRETE FLOOR. EE STRUCK THE BACK OF HEAD ON THE FLOOR RESULTING IN A MILD CONCUSSION.
The employee twisted left knee while walking to the roof bolter. Employee was given restrictions on 5/25/17 that do not allow employee to fulfill employee's normal work activity.
Employee was walking in the #2 entry on 27 Right Section, when EE stepped in a dip in the bottom and twisted EE's right knee. Employee started missing work on 2/21/18 and stated that EE had to have arthroscopy surgery to repair a torn meniscus in EE's right knee.
Miner was crawling in muddy conditions and felt pain in left knee. After saying it would be ok for a month, had an MRI done to find torn meniscus. Worked up to 7-14-17. Surgery 7-18-2017.
Employee was standing next to a gear box that was on a pallet when the box rolled off and hit ee in the left leg.
EE was moving large bottle of acetylene from storage area for delivery. While transporting large acetylene bottle EE lost grip on bottle allowing it to fall and contact employee on left shoulder. EE was transported to clinic and flown off site for evaluation.
Rock was stuck on shaker pan and cat walk employee used pry bar to loosen and when it loosened employee crushed finger between pry bar and railing
Miner was operating a twin boom bolter installing roof bolts at the face when a rock 30""x15""x3"" fell from the roof onto EE's right ankle.
Employee stepped backwards off a platform and rolled ankle.
A fully trained miner was walking across an overcast ramp when the bracket holding the ramp failed causing the miner to fall and sprain left ankle. The incident was not the result of any compliance issues nor were any protective guards involved.
Employee was bolting roof when a rock fell from roof on rib side and struck right foot. Employee was taken to hospital and had a contusion on foot.
While descending stairs, on bottom 2 steps, fell forward bruising knee and scraping right forearm/elbow. Injury was not reported and no medical treatment was given. 9 days later, arm became infected and required hospitalization to heal infection.
The employee's right elbow struck an I-beam causing a stinging sensation. This incident was initially a worker's comp claim only and not reported to MSHA. Surgery was performed on 2/16/18 after all other medical treatments failed to eliminate the pain.
Employee was riding down #3 belt on a 4 wheeler. The ride dropped off the tracks and hit a hole causing lower back pain.
Operator was hooking up super sack with rubber strap. Pulled on rubber strap while stepping off of scale. Lost grip on rubber strap causing strap to re-coil striking operator in the right eye resulting scratches to eye, small cut above the eye.
Employee stated was crossing the #2 belt and when stepping down on the other side of the belt felt pain in left foot/ankle. The employee continued to work until 5/9/17, when employee was diagnosed with a strain, and taken off work. The incident became reportable on 5/9/17 when the employee began missing work.
Individual was exiting cab of P&H crane and caught right foot in the process of climbing down and then fell to the ground.
Employee was torching a steel plate on the Return Deister deck. This area has a 2 deck screen bed where screens are changed out weekly/bi weekly when worn. Torching and welding when needed. Conditions were windy that day and while torching a piece of slag went into employees ear. Plant was down for maintenance and all PPE was worn.
The operator was walking in the tracks the loader that employee had just climbed out of. The loader tires had compacted the fresh wet snow that created a slick icy track and the operator slipped and fell on the compacted snow.
Was looking for oil leak around CM pan and accidentally set CM pan down on foot.
Employee was attempting to loosen the lug nuts on a two man ride. Upon loosening one of the lug nuts the employee felt sharp pain in the mid-lower back.
One employee was tossing down a 100' tape measure to another employee who lost it in the sun and was struck in the face. The employee fractured a small bone in the nose. No work time has been missed. The diagnoses of a need for medical treatment was made on 06/26/17. Repair procedure has not yet been scheduled.
Employee on drive crew was assembling a skirt belt and caught ring finger between an upright and a beam
Employee was attempting to close a misaligned latch on a utility bed tool box with both hands when ee heard a pop in left shoulder.
EE was exiting bobcat and stepped and turned to a locker raising their head up, right cheek came in contact with a stored 2x6 and damaged a tooth. The dentist pulled the tooth and placed the EE on restrictions for 2 days. PPE was not a factor. This is a new EE and EE is very tall. The stored lumber was always previously considered ""overhead"" and out of the way.
Employee was checking engine oil dipstick, felt sharp pain in lower right side of back.
An employee moving a set of rail cars to a spur track, stepped off of a train car from the built in ladder designed for the car and felt a pain in ankle. EE did break both the bones in lower leg. The contributing factors included housekeeping on the surface that EE was stepping down onto and failure to complete a pre-job hazard analysis.
The injured was making an exam as another mantrip was trying to squeeze between the examiner's ride and the rib, catching the ride and bumping it forward. The bumper struck EE's left knee. EE continued working, finished EE's shift and did not seek medical treatment. Became a lost time after receiving a ""no work"" restriction 8/17/2017 pending surgery to repair a meniscus tear.
***6/14/17 - FIRST DATE OF MEDICAL TREATMENT*** Had a 50-pound bag on shoulder & a loading pole in the other hand, went to turn around, foot slipped on uneven ground, left foot went forward & fell backwards to the ground - hurt low back
Miner was injured while moving a conveyor. The tail pulley end raised off of the ground slightly picking the miner up and allowing ee to fall about 2 feet back to the ground spraining rt ankle and left knee. Restricted duty case, follow up in a week. Miner may return to restricted duty on 05/25/2017.
Truck was padded with loose material for a boulder to be loaded. When shovel swung over the bed to load the boulder, the boulder fell through the bucket door onto the back of the haul truck bed. This caused the truck bed to pick up and fall back down jarring the truck driver.
Mechanics were replacing the seed pipe to the # 7 Ball former on the 2nd floor of 426. Pipe rotated out of position. Mechanic tried to arrest the motion of the pipe and felt a pain in the lower back. Member given restrictions and Rx medications.
The employee was helping an over the road truck driver dump some material from an end dump trailer. When they were done, EE tripped on the pile and grabbed the back of the trailer to steady self at the same time the driver latched the tailgate. EE's hand was caught under the tailgate latch crushing a bone in the center of EE's hand.
Employee was shoveling material in the Finish Plant for approximately an hour when the left wrist became aggravated with a tingling sensation and numbness. This incident was managed as a first aid initially, 5/22/17, with little improvement. The employee has been approved for physical therapy 06/13/2017 and has work restrictions due to the left wrist.
Employee was pushing bags of blasting agent in a drill hole with a tamping pole and as EE was removing the tamping pole EE felt discomfort in EE's left shoulder. Began RICE for a few weeks, the employee went to the doctor on 06/01/17 and was diagnosed with tendentious. Employee was given an injection of anti-inflammatory and placed on a 10 lb. work restriction.
Empl hurt back pulling on a strap connected to a poly pipe water line. EE was taken to the ER May 20 and released with no restrictions. June 8 ee was taken to Dr.released with no restrictions. June 10 ee informed supervisor ee could not perform work duties. EE was taken to Dr. June 12 and ee changed June 8 paperwork to show restrictions that made a recordable injury
The miner was doing shaft work and had gone to the bottom to take a break. When ee returned the crew had moved the cage up one level about 10' so ee took a different route up to join the crew. EE climbed through some timbers and framing and lost grip falling back about 2' hitting a piece of iron work. EE was wearing proper PPE.
Employee was walking down ramp and slipped and felt a pain in left knee. Employee went to the dr on 5/22/17 and was released back to full duty. Employee then requested a second opinion. MRI was done and employee has a torn meniscus. Employee had surgery on 9/6/2017 and became a lost time on that date.
EE was working on the 1812.2 Clinker Silo Dust collector fan. EE released the button and as the motor was winding down EE went to see if it had normal air flow. On the way by, EE's fingers came in contact with the moving belts. The tip of one finger was removed and the other was injured. EE reported to the control room and was then taken to the hospital for treatment
EE was in the process of setting an 8800 hydro cone top shell (25 Tons) for manganese prep, EE placed EE's right hand on the maintenance stand while kneeling under to visually inspect opposite side clearance. Once clearance was established, the EE proceeded to set the top shell down while inadvertently keeping EE's hand on the stand, crushing EE's right thumb.
EE slipped and fell coming out of a skid steer fracturing elbow.
The employee was attempting to roll a piece of 12""x24"" 13 gauge sheet metal in a sheet metal roll bender. The employees hand was positioned too close to the rolls which hand went through. This resulted in thumb being degloved and index finger broken.
Employee was stepping down from shaker platform when left knee gave way and ee fell to the ground.
The employee was exiting the administration building through the back single basement door. EE turned to the left to prop open the door; As they rotated to the right to walk to the truck, their right boot caught on the grating. With the forward momentum and the boot caught, the employee fell forward landing on their right arm.
The miner was loading cribbing material, 30-40lbs per, into the bed of service truck when the rope handle attached to the cribbing wrapped around the tip of right middle finger. The miners right middle finger from first joint to tip was broken and degloved.
Operator was hand stacking tube sand and said EE hurt back/bulging disc while performing the job.
Employee was bending down to start wiring a panel, upon attempting to go from a bent over position to a fully upright position while twisting, the employee felt a sharp pain and heard a pop from employee's lower back. The employee strained employee's muscle, fascia and tendon of employee's lower back.
First day of restricted duty 5/30/17. Employee walked outside of east door of compressor room and twisted left knee. There was wet material on the concrete from last nights rain.

Injured was assisting changing a head pulley bearing when they pulled on a pry bar and felt pain in the shoulder. Incident was reported immediately but did not become reportable until 08/24 when an MRI was performed and physical therapy was prescribed.
EE was poking the roaster 4th hearth. EE pulled the poker out to look in the hearth. EE left the poker on the door ledge & took the right glove off to let their hand cool off. EE dropped the glove & bent over to pick it up. EE placed the hand on the hot poker, caused a burn to their fingers. Received 2nd degree burns to fingers.
Lost grip of Hydraulic ram
EE was moving a section of a drop chute out of the maintenance shop down a ramp. The wheels made contact with a pipe; section of the pipe was holding the trough for a screw conveyor. The load shifted and a hinged door swing forward and struck EE's thumb between chute and door, crushing it.
The injured employee and two others were using hammers and speed loaders to install rivets on a mechanical splice. As another person was swinging a hammer to seat rivet, injured employee reached to reposition the speed loader and was struck on the back of the right hand.
Employee was walking from the head side to the tail side on the longwall face. EE noticed that there was a 2 foot piece of belt stuck between the #31 shield and #34 shield. The employee was cutting the belt in smaller pieces and moving it out of the walkway on the longwall face. In the process of pulling on a piece of belt the employee felt a pain in lower back.
The Shovel operator was loading the haul truck on right side while making second pass into bed, the dipper door opened up contacting the duck tail and causing the truck operator to feel pain in EE's neck.
Operator was closing 14"" vapor valve on outside of heater deck on west side of 340 area during a routine heater swap. EE felt a strain in left groin area. Nurse came to plant site and a decision was made to bring ee to the emergency room to get further evaluated.
Employee was moving a motor by hand on a trailer and pulled a something popped in right forearm. EE was diagnosed with a strained muscle. EE started missing work on 5/15/2017.
Driver had removed the 3 side panels from the stake bed delivery truck and leaned them against the opposite side of the truck. A strong gust of wind caused the stacked side panels to fall and strike the driver in the back, causing the driver to fall to the ground from the truck bed. EE broke EE's left arm between the shoulder and elbow and had other minor abrasions.
Injured was checking curtains at the start of shift when a piece of draw rock slid off the side of the fence and struck the injured on injureds lower back. Injured was taken to hospital where it was discovered injured had a transverse process fracture in injureds lower back.
WHILE OPERATING A POWER HAND DRILL, EMPLOYEE WAS STRUCK WITH FORCE IN THE LEFT SIDE OF EMPLOYEE'S CHEST WITH THE DRILL HANDLE WHEN THE DRILL BIT GOT STUCK IN THE HOLE AND THE DRILL TURNED TOWARD EMPLOYEE. EMPLOYEE RECEIVED A CONTUSION TO THE CHEST WALL.
On 5/11 an employee was working in the welding shop and felt something fall on the back of neck from a light fixture above. EE thought the light was going to fall so ee quickly jumped out of the way and right foot was caught causing to trip and land on the inside of right foot. EE felt right knee become sore. On 6/5 ee saw a doctor and put on restrictions.
Employee stated was working in the 3 Left Crosscut, installing the first roof bolt of the day. As EE pushed the bolt into the hole, EE felt a burning sensation in the left shoulder. This resulted in a shoulder strain.
While walking the area to inspect the screen a mechanical failure caused a guard to contact the employee after the flywheel contacted it knocking the employee backward into the adjacent screen.
Work at #3 head frame, bent over to pick up brake board - felt pain to lower right back and right leg.
Employee was aligning a 6"" clay valve and strained their back.
Employee was descending steps of truck dump when the 2nd to the bottom step broke causing them to hurt their ankle.
Employee bent down to pick up a 1 pound piece of cribbing. When the employee bent over the employee felt a strain in employee's lower back.
Employee was at the front of a haul truck talking to the driver on a foot wall lateral that was stopped, engine off, & park brake set when a loader bumped the rear of the truck causing truck to move slightly forward making the front bumper contact the employees left knee. Seen by site medic, referred to ER where restrictions were issued.
Employee was using a fence post driver to pound a fence post into the ground. EE lifted the post driver too high, and it came off the fence post. The driver went above EE's head, and EE lost EE's balance. EE fell down and the driver hit EE in the head. It took a little skin off EE's head, and EE was diagnosed as having a concussion when EE went to the ER.
Employee slipped on berm and fractured wrist.
Re-entering a conduit on a continues miner. While pulling on the conduit ee felt a sharp pain in left shoulder
5-8-17 EE was walking across top of #2 washer tank, surface deflected. EE stated felt a pain in left knee. Sent to medical, ice applied to knee and OTC anti-inflammatory taken. EE working no restrictions. EE sent for follow ups including x-rays, CT and MRI on 6-16-17 found potential left knee ligament tear. Restricted duty Follow up with Orthopedic.
Electrician was manually moving de-energized trail cable when EE felt a sharp pain in left bicep. EE did not wish to seek medical attention at that time. On 5/12/2017, the electrician was in pain and asked to see a physician. EE was diagnosed with a left bicep strain. Restrictions and physical therapy made this injury become recordable.
The employee stated they stepped into a hole with their right foot after dismounting the belt at 33 Break on 4 North Belt and felt pain in the right side of their lower back. The employee sought medical attention on 5-6-17 but returned to work their following shift. The employee again sought medical attention on 5-30-17 and was placed on restricted duty.
The employee was prying on a roof bolter material pod when the employee felt some pain in shoulder. The employee continued to work until going out for surgery on 9/27/2017.
The employee and another employee were carrying a new screen up to the building. When they got to the top of the catwalk and stopped for a break employee noticed an uncomfortable feeling in employee's abdomen.
EE was attempting to change a conveyor torque shaft on the Joy continuous miner, when the shafts cap was loosened. The cap blew out spraying EE on the hands and the right side of face with hydraulic oil. EE received 1st & 2nd degree burns to the top side of both hands and right side of the face.
Employee was pushing stone into a breaker when the stone stopped and right hand twisted around the stone resulting in a sprain to right wrist.
Lab employee was opening a sample jar and felt a ""pop"" in wrist. Did not feel pain at that time but over a few days began to experience pain. Being seen by a doctor for possible cartilage or repetitive.
Employee was walking down an area where rock had been quarried and ground was unstable when they slipped down and fell back on their hands. They injured their right hand due to trying to catch their fall.
Employee was in the process of guiding the #598 production truck out of 3 south stall at mes. The employee was checking the surrounding area on the south side of the building to allow the truck out of the shop. The employee stated that employee slipped on ice and fell on employee's right side.
Employee was helping replace a conveyor belt when the vise grips attached to the conveyor belt EE was pulling on came off of the belt causing EE to fall to the ground on some rocks.
EE was working on the crushing operation when ee suffered a fall from approximately 2' above ground.
Pulling a cable along the framework of conveyor and stepped in hole in grating. Associate fell and impacted knee, back and arm on grating.
A Getman tractor operator pulling an empty trailer ran over undulations in the floor and struck head on a newly installed canopy. EE thought they were fine and continued but said the pain worsened and neck stiffened so they notified the supervisor. Employee was taken for medical examination and released to work full duty but on 5/9/2017 began working under restrictions.
Walking along a belt line. Stepped in a hole hyper-extending right knee.
Employee said while preparing to move the roof bolter employee picked up the cable and shoulder popped. Employee continued to work - an MRI was done on 7/17/2017. Initial diagnosis included degenerative issues, tendonitis. Later diagnosis was a rotator cuff tear related to the incident. Employee continued to work until surgery on 11/7/2017.

Ee was rolling up a cable reel and left thumb became caught and was crushed. Seen at ER, diagnosed with open displaced fracture of distal tuft, distal phalanx of thumb. Referral to orthopedic surgeon resulted in surgery. Ee lost time.
Employee was trying to pry off copper sheet that was still attached at the bottom of the blank. EE put pry bar through the top of the sheet and went forward with force and when the copper sheet released it put the employee in the line of fire for a pinch point. EE pinched 4th finger of left hand between the sheet and the pry bar.
Employee was manually handling mill parts when employee strained employee's Left Biceps muscle
Employee was attempting to remove a bushing from a sheave positioned on the ground. While repositioning to its side the sheave began to slip and the employee got right index finger caught between the sheave and adjacent structural steel support. The right index finger was pinched and caused a laceration that required 10 stitches. EE was released to returned to work.
EE was setting up a stacker. EE's left thumb got caught in between the base and the kingpin assembly after EE loosened the support bracket used for transport. EE's left thumb was fractured and required surgery.
Employee was picking up a stone next to a pallet when another stone fell off the pallet and struck employee's hand resulting in a laceration to employee's left pinky finger.
Employee reported that employee hurt employee's back while installing a plate.
The employee slipped as employee was coming down the front set of steps on the 777 Hauler. The slip resulted in a strained back.
Employee was turning on water system that feeds the crusher for the season. The system consists of a pump and eight inch PVC piping. When another employee turned on the pump an eight inch tee failed causing water to spray in the direction of the injured employee. The injured employee took off immediately when the pipe failed and fell and twisted left knee.
IE was in the process of putting a sling away when it twisted. Felt pain in shoulder which had been surgically repaired in the past. As a precaution IE requested to be seen at ER. No diagnosis given to date, however, employee placed on restrictions.
Employee standing on elevated platform pulling 1B filter cloth to straighten out on unit. While pulling on filter cloth lost grip of cloth fell backwards from platform approximately 5' in height to lower level. Landing on back.
EE was carrying items through a man door and had made two prior trips and on EE's third trip EE was carrying a chain over EE's right shoulder and carrying C grabs in EE's hand while walking from six foot high area through a four foot high area when EE felt a pain and heard a pop sound in EE's lower back.
Employee lifted bag off conveyor belt and back of thumb grazed sprocket on row pusher of conveyor. Conveyor was not working at time.

Alleged: employee next shift stated to nurse, EE's back is sore, that EE lifted a box of bolts the previous day. No report was made to management on date of alleged claim. Employee has long-standing documented physical health matters.
Employee was welding on 4110 Shovel on the left hand side of bucket. Slag fell onto the employee's left metatarsal boot, under the right side flap for laces. The hot slag burned through the top of the boot and sock, causing burns to the top and right side of EE's left foot.

Removing catwalk from conveyor. The conveyor had been set on the ground. Employee cut the bolts holding the catwalk in order to remove it. They then began to remove the remaining pieces of the bolts that were still supporting the catwalk. When the last piece was removed the catwalk shifted and pinned the left hand against the frame of the conveyor.
EE stepped on a board and twisted EE's ankle. After being seen at the doctor's office it was determined that a bone in EE's ankle was broken resulting in restricted duty.

EE attempting to clean at #5 crusher train dump when EE lost footing and fell into open hole. EE received sutures in a laceration on bottom lip and was placed on restricted duty for knee pain.
Employee was driving a loader across the yard with the bucket lifted, speeding, and hit a spillage pile on the flight side of loader causing the loader to go left on two wheels. When that happened EE jerked it back to the right side, hit the ground and the loader continued in that direction and fell over.
Miner was moving railcars remotely from the ground when the gravel under feet shifted causing ee to loose footing and fall, injuring back.
Inside clinker cooler, backing and turning walk-behind skid-steer machine and machine turned faster than normal due to metal grates and clinker material. This caused skid-steer to catch employee's right knee between clinker cooler inner wall and machine resulting in swelling and restricted duty (soft tissue injury).
Employee was attempting to drill out a stripped brass fitting. While starting the 1/2 inch hole the drill slipped off the gauge and contacted the webbing between the employee's thumb and index finger on left hand. The drill bit made about 1/2 inch cut.
Employee was removing table liners from the coal mill. One of the liners had a strap around it so that it could be lifted with the hoist. The employee was taking up slack from the choker when the liner tilted and fell over, striking the employee's leg.
Hit a bump while operating the front loader and says ee hurt back/neck.
EE was removing a drill rod from a drill. EE was holding the drill rod on the very end instead of the middle. When the threads finished the rod fell to the ground jolting EE's left elbow. This inflamed tendonitis in EE's elbow. EE was shown stretches and released to full duty until the injection on 6-13-17.
Employee was descending rear ladder on a rail car when EE slipped on the next to last rung (approx. 5-6') landing on EE's back. It had been raining and the area was muddy. The employee was wearing a fall protection harness and tie off to a retractable lanyard.
Employee was walking down from a rail switch and turned ankle on the ballast rock.
While entering lab, employee stumbled on metal grating and fell forward using both hands to brace fall which caused cuts to both hands and finger. Employee was taken to local Urgent Care for observation and diagnosed with laceration without foreign body to both hands and finger and received a total of 13 stitches.
Employee was putting a fork on the forklift when they lost control of the fork and it fell striking their right foot. The blow caused a small cut on the top of the right foot just above the steel toe on their boot
Employee was working on Portable Crusher and was struck by a blunt object resulting in an abrasion and bruise to the lower back and pain to EE's torso.
Mechanic sustained injury to fingers while up righting CAT #4306 electric pallet lift when hand contacted a pinch point. Employee was evaluated by site responder and nurse. Employee was brought to healthcare facility for further evaluation and treated for finger tip fracture. Safety department initiated investigation into the incident with support from the department.
Employee was in the process of moving the C/M machine from the #2 entry to the #1 entry. In the process of moving the employee stepped in a hole twisting right knee. The employee scheduled a doctors appointment for 7/23/2018 and received two cortisone shots.
Employee was bending a roof bolt when employee felt a pop in employee's right forearm causing a sprain to employee's muscle.
An employee was walking in the parking lot and tripped on gravel, twisting right ankle.
Employee required stitches after contacting an overhead beam. Employee was also placed on a lifting restriction for one shift.
EE strained while moving a partial drum of mill balls, physician diagnosed as thrombosed external hemorrhoid. Medical treatment to lance and drain, and restricted duties.
Employee was using a crescent wrench to loosen a nut when the wrench slipped off the nut causing ee to strike finger against a hand rail support.
Employees was climbing steps to access top floor of milling area, while making a turn on second level platform EE's right knee buckled. EE stated the right knee was bothering EE on 5/21 and was seen by a doctor on 5/22. Doctor put EE on light duty and a follow up appointment was scheduled.
The employee was not wearing safety goggles while dispensing grease to a Trackhoe. The hose burst and entered the eye of the employee. Employee drove to the emergency room on own due to denying assistance. No medical report was received. The employee is seeing blurry and has not returned to work.
Employee was positioning a conveyor pulley and had to push against pulley for final position. EE felt a pain in right shoulder. On 7/11/2017, ee received restrictions of light work.
Employee was attempting to get a strap around a broken blow bar when the bar dislodged from the backing and pinched left thumb against a rotor. EE went to have an X-Ray and shows a potential hairline fracture of thumb.
Employee was walking backwards in the maintenance shop. While doing so EE tripped over a pallet and landed on the part that was on the pallet and hitting EE's tail bone.
Repairing a screen and cut right thumb which ee needed stitches.
Employee was lifting a piece of limestone/Wallstone a conveyor on to a raised pallet. Muscle strain in groin area
Employee strained elbow while opening elevator door in mill. Saw doctor day of incident. Treatment at that time was not reportable. Employee still experienced pain in elbow. Returned to doctor on 6/5/17 and was prescribe therapy and light duty for elbow.
Was pulling on a safety cable out of the water to attach to the dredge, when EE felt a pop in the right shoulder. EE went to hospital where it was determined that EE strained the shoulder.
EE was climbing EE's loader normally and felt a pop in EE's shoulder. EE was taken for medical treatment and has been placed on light duty restrictions.
Two employees were lowering metal screening roll down from rack in storeroom. EE stated the material was awkward and heavy. EE turned wrist while lowering the material. EE told supervisor of the incident, but did not feel it needed further action. The EE administered home first aid. On 5/18/17 the EE told supervisor symptoms were worse so EE was sent for medical treatment
Two employees moved the wagon through the narrow pathway with a 500 lb tail pulley. The bottom of the wagon struck a bracket protruding from the floor. While pushing the wagon to free it, the tail-pulley rolled off wagon, Employee 1 fell backwards into the roll press door, and pinned Employee 1's left ankle between the pulley and the removed roll press door.
Driller's Helper was lifting a core barrel off a storage rack when they pulled a muscle in their lower back.
The employee was operating the loader in the plant when the bucket dropped suddenly stopping the loader short, which propelled employee's body forward, causing muscle sprains.
EE was assigned clean-up work on Belt #9. The weather was windy with strong gusts. When leaving the MCC after locking out, a strong gust of wind blew material into their right eye. The EE was wearing protective eyewear.
EE was pulling on a wrench to loosen a cartridge when the cartridge broke loose causing EE to hit elbow on the fender. EE was placed on restricted duty on 6/14/2017.
Miner was inspecting the train before loading. Miner was dismounting off of a piece of equipment when an article of clothing got caught causing the miner to fall. Miner received a cut to elbow and a bruised hip.
Employee stepped into hot liquor causing a burn on the right foot.
Employee was standing on 2 pallets when ee stepped down and right boot landed on a rock which rolled resulting in a sprained right ankle. Employee has been on light duty since the injury and is projected to continue through 05/29/17. No lost time.
EE pinched their hand between hoist cylinder and the trailer frame when trying to remove the cylinder.
Miner was operating a man lift. While extending the boom the right thumb was pinched between the hand rail and a I-Beam on the Stedman crusher.
Employee was operating 772 haul truck and the employee smashed middle and ring finger on right hand between the frame and the seat.
EE was moving a hydraulic ram in a storage connex and felt pain in lower back. EE was evaluated and put on modified duty.
Employee stated ee was operating the diesel scoop when the machine overheated. EE heard a leaking noise from the rear of the scoop, and as ee walked toward the rear of the scoop to see where the noise was coming from, the cap covering a reservoir blew off. This caused hot fluid to contact the side of face, resulting in burns and restricted work days.
Employee was returning a derail device to its storage room. When ee went to put the device down ee experienced back pain. Diagnosed as back strain.
EE was descending the Water Truck with both hands free. EE stopped on the bumper step with both feet to relocate lunch box to the front bumper with left arm while maintaining 3 points of contact. EE slipped / fell 32"" to the ground landing on both feet in the catchers position. EE fell forward hitting right shoulder on the lower step support bracket.
Operator was lying down on a roller platform to spray out sand underneath a load out scale. Upon standing, operator twisted knee. Operator felt discomfort & reported it. Operator was released to work by operator's physician without restrictions. Upon further evaluation by operator's physician, surgery was recommended. Operator returned to work after surgery with modified duty.
EE reported injury on 5/11/17 but did not want medical evaluation until 5/12/17. EE stated while walking during shift felt a sharp pain in lower back. EE cannot recall anything they did that caused the pain. EE taken for medical evaluation and placed on modified duty for 4 working days.
Employee was making a water run and drove the flat water fleet truck to the bulk water tank. Upon stepping out of the truck, employee stepped down with employee's right foot and immediately rolled employee's ankle on uneven ground/surface (small raise or mound of dirt).

Employee had a hole in their pants. Finished products of GCC (cement)got into employee's pants and burned their leg.
EE was working on the #2 Filter feed pump. EE was behind the boom truck walking south from the #6 sump on the concrete pad to hook up the motor when ee slipped and fell on side impacting shoulder. Splint was applied and was given restrictions.
An employee was performing work on the plant front-end loader. Employee was pulling on a wrench to loosen a bolt when employee felt a pop and a sharp burning pain in employee's right arm.
The principal injured was walking the return air course in Sulfur Mains when they heard the mine roof give a break away from them. Their reaction to ""run"" resulted in them striking their head (hard hat) on a crib resulting in pain in their neck. They continued to work until 5-17-17.
A GMS employee, working on a GMS specialty project at Sufco Mine, was unloading a trailer of ball busters. One broke loose and employee smashed finger between two ball busters. GMS crew leader was witness.
Operating loader, hit a hole and strained upper back.
EE was trying to get an 8'x4'x 5"" rock off of the continuous miner. EE grabbed the rock with hands and was trying to help the continuous miner operator remove the rock. During this process the EE stated EE had hurt EE's back.
A Utility Maintenance person sustained a sprain of left shoulder from a fall on the same level. The injury occurred while the injured and a coworker were pulling a new wire rope into place for the bow spud on the plant with a 3/4"" rope. The connection between the ropes did not hold resulting in the fall.
Employee was loading on top of rail cars. Employee was tied off to fall protection the entire time. Employee was using a retractable lanyard and let it trail behind while walking on the car and moved too quickly causing the retractable lanyard to lock. Employee lost balance and fell onto the catwalk on the rail car and sustained injury.

EE was walking toward a pump station when ee tripped over a 2"" water hose, causing ee to stumble and land on wet sand awkwardly with left foot causing a six inch crack in left fibula.
Employee stepped off of one barge to another as they turned to walk down the barge they tripped over the loose dock line causing them to twist an ankle.
EE was standing on bucket when it collapsed. EE fell to the ground twisting left knee. NOTE: Ladders are provided. EE chose not to use proper tools for task.
While cutting lifting eyes with a cutting torch, the metal fell off the table. The employee reacted and grazed left shin with the torch causing 1st and 2nd degree burns to an area approximately 3"" x 5"".
A Production Attendant was in the process of opening the Tertiary Air Duct elbow door when one of their fingers was caught in a pinch point. The employee sustained a cut to their right ring finger.
Principal injured was attempting to remove a stuck drill steel from the mine roof by raising the drill head to the roof to align the wrench with the stuck drill steel when employee's right thumb was caught between the wrench and the T-3 channel strap employee was bolting to the mine roof.
Worker tripped on a protruding piece of expanded metal that had been propped against the wall at the working area, near no.1, raw mill separator. Worker fell onto the expanded metal, lacerating the little finger on the right hand, requiring stitches.
Contractor EE was operating a roof bolting machine, while installing a fly board to the mine roof, the fly board flexed and sprung upward, pinching the EE's finger.
Mill Ball fell from above and struck employee on hand, bounced off and hit knee
IP stepped off of water truck after it was parked. Using three points of control IP stepped off of placing left foot on the ground. IP then rolled ankle causing discomfort.
Hopped over narrow drainage ditch and slipped in mud.
Employee was checking rotex screen for possible plugged chute. As the screen was stopping, employee placed hands and right foot against the screen to stop it when the screen shifted causing employees foot to slip in between the screen and the chute resulting in sprain and bruises on right foot/ankle.
EE states was cleaning a conveyor table with a T-rake, the T-rake got into the roller pulling EE's hand into the roller. EE went to the ER, X-rays were taken and all was negative and no restrictions. EE went back to our work comp clinic on May 17th and was given restrictions to also start physical therapy on May 22nd.
Employee was moving trail cable for dragline when ee felt a sharp pain in lower back. Employee was taken to urgent care for medical attention was diagnosed with lower lumbar strain was given anti-inflammatory prescription and a prescription for pain.
EE was removing rocks from the tongue of the trailer to hook it up to the tow vehicle. EE picked up a rock and the tongue moved in an upward direction, smashing the employees thumb between the rock and the tongue on the trailer. EE was seen at the ER and diagnosed with a fracture to the thumb. Released with restrictions.
Cleaning flok room floor with 4lbs hammer and struck left hand.
EE was advancing rock dust hose when it caught on belt structure causing EE to strain their upper left arm and shoulder. The EE said they were having pain on 5/16/2017 and evaluated by a Doctor and prescribed PT. EE started PT on 05/22/2017. EE is now a Medical Treatment accident.
Employee was removing liners that were left over from the 43 feeder job. EE was moving liners to the south wall of the crusher. On 5/3/2017, EE was diagnosed with a hernia.
While attempting to mount a Cat 777D, an employee with 2 years of mining experience had the wire rope ladder swing forward towards the vehicle causing the employee to fall downward with right foot still on the rung and landed on ee still extended left leg. As a result the employee suffered a hyperextension of the right knee.
Miner was removing metal grating segments, which weigh in range of 25 to 50 lbs, from the process ditch when miner had a sudden sharp pain in miner's lower back.
EE was sitting down in a chair and the chair broke causing EE to fall to the floor hurting their back.
While removing a valve, a mechanic accidentally bumped a 24 inch pry bar, which fell through a grating opening, striking the pipe below, then falling at an angle, striking an employee working approximately 15' to the side of the overhead work, resulting in a non-displaced curvilinear fracture of the scapula.
The crew was changing an anchor wire on the Dredge IV when the load shifted unexpectedly. The shift causing the lifting equipment to become taut thus causing a recoil action on the chain. The associate was struck in the lower portion of the facial area. The associate received a small cut on the left cheek and lower lip.
EE was installing rib support on the working section when a piece of draw rock fell from the mine roof, striking EE in the left shoulder.
Operator was securing a pallet on a shipping flat when the chain being used slipped causing ee to stumble on flat ground, resulting in a rolled ankle. Initial pain subsided. The next morning (Tuesday 5/2/2017) ankle was still an issue. Flew back to town and had x-rays and MRI performed. MRI showed damage to a ligament on that ankle.
Employee pulled on a spreader bar in the back of employee's truck to roll it off of a bucket and felt a sharp pain in employee's left middle finger. Following that, a soreness persisted followed by numbness in the fingers/palm.
Chipping off clay from a track with a pry bar, missed track and hit left pinky finger on the track.
REPORTABLE ONLY DUE TO SUTURES - Employee was changing a 3/4 inch hose inside a shield on the longwall, when the tool employee was using to remove the staple slipped hitting employee's lower lip causing laceration requiring sutures.
Employee was replacing a bearing on a highwall miner add car, when the bearing that employee was turning suddenly turned pinching employee's left index finger. Employee was wearing protective gloves. This resulted in an avulsion to the tip of employee's left index finger.
Crusher employee was opening overhead door. Wind blew dust and debris into the shop area. Employee got dust in eyes. Flushed eyes with eye wash solution. Left eye still felt sore, safety rep brought employee to the eye dr. There was a small foreign object in employees left eye. It was removed and antibiotics were applied. Employee returned to work with no restrictions.
Employee was stung by a bee.
On 5/31/2017 Employee reported an injury that happened while loading mill balls. Employee got hit on the lip with a piece of metal. EE was released back to work full duty same day. On 6/17/2017 employee felt something in EE's lip. We took EE to the clinic. The doctor sent EE for a new x-ray that showed something in EE's lip. On 6/22/2017 they cut it out and put in 1 stitch.
Employee had just finished putting up a roof bolt and turned to re-load tray with material. When ee turned,ee then struck the back of arm against a stack of roof bolt plates. It caused a laceration about 1"" long that required 4 stitches. Employee returned to work after stitches were put in.
while moving broken baffle out of coal line, a sharp piece of angle iron came in contact with employee's top of hand causing a cut.
While handling a bundle of 8 foot roof bolts the employee felt sharp pain in right shoulder. EE began missing work on 8/15/2017 to receive treatment on the shoulder.
IE was removing a cell from a coal hauler battery. The cell top became delaminated causing a gap between the cell top and cell casing. The cell slipped out of IE's hands landing vertically on the floor causing acid to exit the top of the cell. Acid splashed under IE's face shield, around the safety glasses contacting left eye. Prescription eye drops were issued/used 5/31/2017.
Hitting a bit with hammer and a piece of the hammer came off striking EE in the hand. Medical Treatment due to receiving stitches.
Concrete finisher was working in the area of a concrete block wall (eco blocks) when a landslide occurred causing the wall to fail. The wall fell and made contact with the workers lower leg. Worker initially declined any treatment and said was ok.
Employee was assisting another employee with the dovetail of a trailer to add an extension to lengthen the trailer when the incident occurred. There is a handle on the dovetail but it was stuck so employees put hands underneath to pull up on it and the stand that is underneath came swinging back pinching employee's left middle finger. This resulted in a nine stitch repair.
Employee was cleaning a filter and got caustic slurry on pants leg. The employee decided to put on rubber boots and tucked the wet pants leg into the rubber boots. The caustic caused the employee to receive 2nd degree burns.
Contract employee was air arcing with a welding shield on and wrap-around safety glasses on and something got behind everything and went into EE's eye. Cause of this incident was possible face positioning. Compliance with rules/regs., PPE, mining equip/systems and/or behavior was not a factor.
Injured EE & helper were putting cathodes on strongback with the cathode hand tools. When the side that the helper was moving slipped from the strongback causing the cathode to fall on the injured EE's side, crushing finger in between the hand tool & the transfer case. EE's supervisor took EE to the clinic where EE received 3 stitches to tip of middle finger on left hand.
Subject using a bar to get a piece of coal out of the crusher. The subject crushed finger between the bar and a metal plate.
Employee was lifting a pump shaft pulley and strained right bicep.
An equipment operator slipped on the dozer tracks while pulling on a pry-bar causing the bar to hit EE in the mouth, chipping EE's upper left front tooth.
Rotating rotor on red mill crusher to check clearance. Employee got their finger caught between the drive belts and sheave.
Employee was heading down the Longwall face from the tailgate towards the Headgate. EE raised up while ee was walking and came into contact with the side of a shield. This resulted in employee receiving 7 stitches.
Employee was using a kiln ring blaster eight-gauge shotgun to remove a ring in the back of the kiln. After approximately thirty shots, three small pieces of material or shotgun slug projectile ricocheted back and struck face resulting in two sutures on the bridge of nose.
Crew was moving a temp electrical panel for crane access during a clean-up of a jobsite. While sliding the temporary panel along the grating, panel base became lodged on a cover on the grating. EE stepped in to help the crew, placing hand between two objects. As the crew maneuvered the panel, it jumped and the injured EE's hand became caught between the temp panel and a pipe.
Slipped and fell on track while climbing off CAT 374 Trackhoe. Scraped skin on right shin.
Employee was performing maintenance on palletizer, making adjustments to turntable. Employee pulled on wrench to loosen a bolt on the bearing & felt a pinching pain in neck & shoulder; reported incident to supervisor on 05/30/17 because pain was still present. Employee was scheduled off work on 05/31/17 & went to clinic for checkup (no time was lost from work).
Employee was replacing V-belts on a screw conveyor and got lime in EE's eye.
Dirt/debris entered the injured miners left eye on 05/26/2017. Medical treatment was requested on 06/05/2017 because of a continuing eye irritation. The miner was prescribed eye drops on 06/06/2017.
Employee was breaking stone down when pieces of stone came through the screen and hit employee's cheek and a piece of stone got behind the employee's safety glasses and in eye.
Employee tried to access door on crusher without proper fall protection, in an area requiring fall protection. Employee fell and injured right hand.
Injured was putting rubber matting over leads on B56 motor, while it was located in the main wash down bay of the Metz Motor Barn. While cutting holes in the rubber matting with a utility knife, the knife slipped and EE stabbed self in the left palm, in the meaty part below the thumb. Required 4 sutures, employee missed no work due to this injury.
Employee received injury to finger while using rigging equipment. Resulted in five sutures to right ring finger.
A laborer was tending a bricklayer in an autoclave. The laborer reached into the bucket to retrieve a brick that had been saw cut. The brick cut through the EE's leather and latex gloves. The EE was transported to the hospital where EE received 8 stitches to the left index finger.
Employee was using a 6"" die grinder with a burr bit to clean welds as the employee progressed with welding. Employee got a small piece of metal in right eye.
Employee was closing the lid lock on a batch truck in the 1285 level. While striking the lid lock with a wrap wrench in right hand, the wrench struck the employee's left middle finger. The employee sought treatment and received 17 sutures to close the wound.
Employee was putting up on of the metal roof mats with the roof bolter and as employee was looking up a piece of metal fell behind employee's glasses and got in employee's right eye. The piece of metal was removed and the employee received eye drops and was released.
Employee was using spray paint to paint a center line in the #3 entry on the #1 section. As they painted the center line the air movement in the entry caused paint particles to blow into their eyes. Employee states they were wearing safety glasses. This resulted in an irritation and prescription eye drops. No lost work days.
A piece of rock from a rib corner struck the employees forearm resulting in employee getting 10 sutures.
Injured employee was helping to change a lime screen when some lime dust fell behind employee's safety glasses into employee's left eye. Employee had antibiotics applied to employee's eye.
Injured was putting scoop tire back on 604-1008 scoop, located one heading over from the track of the St Leo rehabilitation area. The tire was lifted in the air with chain hoists & the employee was wiggling the tire back & forth to get it to seat on the wheel studs & caught left ring finger between the rubber tire & the wheel stud, resulting in a fracture & a laceration.
The employee was in the process of put a spring back in a R25 Euclid and the spring slip and pinched finger between the steering cylinder and spring
Driller had placed a cardboard blast hole tube into a completed hole, while cutting to proper height of tube above the ground. EE was pulling EE's cut off tool toward self, tool slipped sticking in EE's right lower leg.
Employee was changing side liner in the impact crusher the plate slipped hitting the ring finger on the right hand causing a small laceration.
EE was closing gate on scrubber when material fell from above into left eye. EE flushed eye then wiped it; flushed again. EE was re-evaluated next day, 5/26, and had abrasion in eye. Prescribed rx for eye.
When the spout at the Hydrate Bagging Machine was plugged the employee went to open it while under pressure and lime released from the spout getting into the employee's eyes.
The truck operator was checking why a hydraulic cooling fan wasn't running. As ee checked the winning behind the fan and plugging in the wires the fan started up. A foreign object broke through the protecting cover over the fan and cut left pointer finger tip of operator, cut was repaired by 6 stitches.
EE was in the process of spraying plaster coating onto a stopping located outby spad 422 in the intake on 1st right panel section of the mine. The spray nozzle became clogged and EE attempted to dislodge material from tip at which time plaster material was released from the nozzle striking EE in the eyes.
An employee was in the process of completing a lockout when they dropped the lock on the grating, bent to retrieve the lock and when standing up, struck their right ear against a sink resulting in a laceration to the mid section of the right ear. The laceration resulted in 5 sutures placed on the outer edge of the ear.
Employee was taking part in fusing the mainline. Chainsaw needed a new blade and the employee was taking it out of the back of the pickup to change it out. The employee was wearing cut resistant gloves, and the hand holding the top of the chainsaw slipped and the momentum and weight of the chainsaw kicked the blade into the employee's lower lip causing a laceration.
A log got stuck in the dredge hopper. EE got supports to block the grizzly from moving. EE was positioning the support by reaching through the handrails. The support slipped, the grizzly kept descending and struck EE's hardhat. EE pulled head back, cutting nose on the handrail.
EMPLOYEE WAS HELPING CLEAN A HEAT EXCHANGER IN PROCESS BUILDING WHEN A FOREIGN BODY GOT IN EE'S RIGHT EYE. UPON MEDICAL TREATMENT IT WAS FOUND TO BE A SMALL PIECE OF METAL. IT WAS REMOVED AND EE WAS GIVEN A PRESCRIPTION OF NEOMYCIN.
Notified today (9/20/2017) the accident aggravated a previous injury. Employee pulled a fuse without proper PPE. Did not see a doctor at the time. In July reported the injury. Has been undergoing numerous tests.
Employee working on airline trying to tighten rusted fitting to prevent leaking. Fitting blew out of housing on main plant air feed line injuring hands. Employee has impact burns and small fragments of shrapnel in hands.
Breaking up rock jam with a bar. Bar slipped and hit employee in forehead. Required 6 stitches.
Employee was operating a JLG scissor lift. As they moved the machine closer to the work area, their right hand was pinched between the operator's basket handrail and a steel beam.
Welder was standing on a platform, driving a wedge setting the sidewall on a dragline bucket in the bucket shop. The employee lost balance and put left hand on the dog while swinging the 6 lb. Sledge. The sledge glanced off the wedge and struck the employee's left ring finger.
During the pre shift inspection on the Dredge EE hit the (top) back side of their hand and EE does not recall what they hit their hand on but does recall while inspecting a dewatering screen they hit their hand on something. At the time they did not notice anything but about a hour later they noticed their hand bleeding.
Operator upset equipment while dumping load. Operator failed to ensure truck was level before attempting to dump. Truck upset on passenger side. Operator was utilizing their seat belt at the time of the incident. Resulting injury were lumbar, lumbar radius strain. Miner refused medical treatment.
Employee was side bolting in E29HG and while raising the steel to the roof a rock came out from the roof, bounced off the strap and struck the employee. Rock hit employee on the left side of employee's face and employee received two stitches above employee's left eye.
While changing the outlet fitting on a trailer mounted pump, the wrench slipped off the bolt and struck employee in the head above the left eye at about the eyebrow level. Employee was treated on site and taken to the emergency room where they received five stitches.
The employee was helping load a roll of conveyor belt. The ""D"" ring, that ee had placed onto the bar that was inserted through the belt spool, slid off and struck ee in the mouth.
Employee was spotting a roof bolt when a small piece of draw rock fell and struck ee on the left hand. The result was a hairline fracture of a metacarpal bone on top of the hand.
Injured employee was preparing belt structure for installation. EE was separating 60"" top rollers when right ring finger was pinched between two rollers. This resulted in small laceration and fractured finger tip.
Employee was removing the dust collector assess door when it fell striking ring finger cutting it which required stitches.
Supervisor was helping another employee when hot material from the coal mill take off shot out at a distance of 12 ft striking the supervisor on the back of the neck and ear. This caused 2nd degree burns. EE was treated at UAB and released back to full duty with no restrictions.
An employee was tightening a hose fitting when the wrench slipped allowing their arm to contact a support for the fan shroud causing a superficial cut on their forearm. The physician applied a steri-strip(r) to keep the wound clean, dry and to avoid infection and not for wound closure.
The employee was hooking up the diesel duster to a Getman and got their left ring and pinky finger caught between the tongue of the duster and the hitch on the Getman. This caused a laceration and sutures were required to close the wound.
Employee was exiting the Guard Shack, and experienced a muscle cramp in left knee, then rolled left ankle and fell to the ground.
Employee was standing in front of the miner, lifting cable and water line off hooks on the side of the pan. When they attempted to lift the cable and waterline they both were tighter than thought, as they pulled their arm they had a ripping feeling and let go. On 6/16/2017 they received stitches due to surgery.
Employee was removing a stick cylinder from an excavator, hydraulic pressure caused pressure release getting employee sprayed with hydraulic fluid on the right forearm.
Employee was lining up a punch and die set in a Piranha Ironworker. In doing so, the set up was not aligned and the punch came into contact with the inner edge of the die. As the employee bumped the machine to recheck alignment the pressure of the punch and die contact caused a small sliver of metal to break off and strike the employee's left cheek.
Employee was using a come along and choker to pull the HDR. The HDR had become stuck and would not move in either direction. When EE pulled with the come along the choker failed and the hook came back and struck the EE. EE sustained a broken pinky finger on EE's right hand.
While putting lid over cable reel, shuttle car cable started to reel up and anchor hit employee in upper lip. Caused a cut in upper lip
Employee was changing a cat head on a roof battery. While cutting the cable the utility knife slipped and cut left index finger.
EE was spotting a corner bolt LOCC #5 entry inby right corner, when a piece of draw rock measuring 39""x10""x10"" at widest point fell and cut left forearm. EE received stitches at the ER.EE has no loss time and return to work next day.
Individual was tightening a T hand on an iron hand attached to a forklift with a wrench. The wrench slipped causing the individual to strike hand on the forklift causing a laceration.
After installing a new floor for log washer chute, a two foot piece of angle iron was tack welded into place and needed to be removed. Tack was small and just needed to be wiggled to break loose. Employee wiggled and pulled back on the weld. When it broke free, the end of angle iron hit right cheek and caused a small laceration requiring two stitches.
Employee was removing & replacing the bucket teeth on an excavator. Employee cut them with the torch and was using a hammer to break them free when a piece of metal struck the employee in the leg resulting an ER visit to remove the foreign object. Released to work the following day.
Employee was removing the gear box from a head pulley. Employee was using a 3 ft 6 lb soft face hammer. EE struck the bearing in an up to down motion. When ee hit bearing it shattered in multiple pieces. One of the pieces struck the employee in left upper arm, approximately 4 inches in length.

Employee fell backwards to the ground while using channel locks to pull a crease out of belt. Employee tried to break the fall injuring employee's left wrist.
While operating a crane a wind storm suddenly popped up. This storm kicked up a bunch of dust in the path of employee. EE was wearing safety glasses and other personal protective equipment. During the wind storm some dust got in the left eye.
While bleeding water line, it came apart striking shoulder. This was a notice only claim until 12/4/17 when ee began missing work.
Employee was operating a electric (walk behind) pallet jack to unload a truck when employee ran over their right foot.
The employee was assigned to work near the head gate of the 12 Right longwall. The employee was recovering a joint of rock dust hose by cutting it off with a razor knife. The employee was holding the hose with left hand and cutting it with right and the blade lacerated left forearm. The employee finished the shift. Later had 3 stitches placed in the arm.
An employee was moving a cut cathode when ee received a laceration to the lower right arm.
IMG employs were assembling buckets to waste elev. chain. Employee went to straighten a piece of chain and finger was caught between 2 buckets. This caused a laceration to the right index finger. The Risk Assessment and JHSA both advised about pinch points. IMG employees signed off at the safety meeting that morning of the risks related to this job. Employee was wearing gloves.
Employee was turning wrench to loosen a bolt, wrench slipped, hitting employee in the lip requiring stitches.
EE was bolting on miner when a piece of rock the size of a fist fell striking the bill of the EE's hat driving it into EE's glasses causing a laceration on EE's right eye lid which required 3 sutures.
While rib bolting off of the side of the miner, employee pinched left thumb between the bolt and the bolter head. Employee received 6 stitches to close the laceration. No lost time.
Two employees were removing a pivot shaft seal using a pry bar and a utility knife. While one employee was prying on the seal the other employee was cutting the seal when the pry bar slipped off and struck the employee that was cutting the seal. Employee received three stitches in nose.
Employee was opening a valve when they hit their right arm against a beam that had a burr on it causing a laceration.
While operating a scoop, employee ran over a wooden 4x4 causing neck and lower back to be jammed. Employee then twisted knee getting out of the scoop. Employee was taken to Dr. and received a full release with no restrictions. On 08/29/2017 employer received notes from WC that employee had received physical therapy.
Reportable only due to sutures. Employee was change bits on the long wall drum when a small thin piece of rock fell from between shields hitting employee on the left forearm.
An employee was walking down the stairs when employee lost employee's footing and fell, sliding down 14 steps to the ground level.
Tighten up pull cable, wind blew barge out and cable smashed right middle finger. Took to emergency room, had 3 stitches put in and back to work.
