An unintentiona roof fall occurred in the #5 entry just outby spad 499. fall measured approx. 20' x 20' 0-5'. fall was timbered off. fall was approx. 11,000 from 013 mmu in an entry of the intake air course.
Hoist was down due to electrical problem in the control panel.
A ROOF FALL WAS FOUND BY THE MIDNIGHT SHIFT MANAGER WHILE EXAMINING OLD WORKS AND EP AREA. THE FALL WAS APPROXIMATELY 60 FEET IN LENGTH AND 19 FEET WIDE BY 5 FEET HIGH. AREA WAS BOLTED WITH 5 FT FULLY GROUTED RESIN BOLTS. FALL AREA WILL NOT BE CLEANED UP AND DOES NOT IMPEDE TRAVEL OR VENTILATION.
AN UNINTENTIONAL FALL OF ROOF OCCURRED IN BREAK # 17, #3 LEFT CROSSCUT IN 1ST LEFT OFF SOUTH MAINS, 002 SECTION. tHE FALL WAS APPROXIMATELY 18' WIDE X 23' LONG X 7' THICK.
CONVEYOR BELT #301 WAS RUNNING IN THE HAND POSITION (NOT IN SEQUENCE) & OVERLOADED THE BELT IT FEEDS - #205. WHEN BELT #205 WAS GIVEN THE SIGNAL TO START, IT WAS OVERLOADED & NOT EQUIPPED WITH A ZERO SPEED SWITCH. THIS CAUSED THE DRIVE TO SPIN IN THE BELT & BURN IT. OUR ACTION WAS TO EQUIP THE BELT WITH A ZERO SPEED SWITCH. DAMAGE AMOUNTED TO RESPLICING A SECTION INTO BELTLINE.

VENTILATION AND PASSAGE WAS NOT AFFECTED AND THERE WERE NO INJUREIES AS A RESULT OF THE FALL. THE PERIMETER OF THE FALL WAS TIMBERED AND DANGERED OFF.

An unintentional roof fall occurred at the millennium Portal 6N headgate unit entry #2, 2938 south. the fall was approx 14' long, approx 9' wide and unknown thickness. the fall did not affect ventilation or passage of people. The fall will be cleaned up.

The encoders were giving false readings int he computer. the miners were evacuated at 5:40 p.m. and was completed at 5:53 pm. the switches were reset and zeroed encoders. the hoist was tested at 9 pm and the miners returned underground at 9:15 pm.
A roof fall occured on unit 002 of the south west submain in entry #3 and survey station 00 & 0. the fall measured 20' long x 30' wide x 10' thick. this area had been bolted with 9' double lock and 6' fully grouted bolts. clean up will comply with 75. 212 cfr
ROOF FALL OCCURRED IN THE #2 ENTRY RETURN MAIN SOUTH AT 66+40. THE FALL WAS IN THE INTERSECTION. 6 FT. REBAR (TENSION BOLT).
A roof fall occurred on the 2L4W, entry 2-3 at cross-cut 38. The cause of the fall was a weak laminated shale commonly called stack rock. The area where it fell is in an area where future mining will occur.
A roof fall occurred one break inby spad number 5152 in the #5 entry on the #5 conveyor belt. The fall measured 4'H X 20'W X 20'L.

A ROCK FALL OCCURRED IN THE #2 ENTRY AT BRK 15. THE FALL WAS APPROX. 40 FT. X 60 FT. X 8 FT.
TALL CRIBS HAD BEEN SET IN THIS AREA, BUT DUE TO THIS AREA (1 BREAK SPAD #3962) HAVING WET TOP - THE TOP MUST HAVE DETERATED BECAUSE OF BEING WET. THE FALL WAS APPROX 20' X 30' X 15' HIGH.
A roof fall occurred at x-cut 127-128 in entry #4 of the 3W mains. The fall covered about 20' of belt stucture. The fall was 20' long, 15' wide and 4' high. The cause of the fall was a weak laminated sandstone and shale mix.
AN UNINTENTIONAL FALL OF ROOF OCCURRED ON 002 SECTION, 6TH LEFT PANEL OFF WOLF CREEK MAINS BREAK #5 AT SPAD # 8917. RETREAT MINING WAS BEING CONDUCTED AND AFTER COMPLETING THE LAST CUT ON THE PANEL THE FALL OCCURRED.
The emergency hoist malfunctioned and would not lower the escape capsule into the mine. An electric motor brake locked up and would not allow the cage to lower. The brake was repaired and the hoist returned to service.
An unintentional roof fall occured on the C-Right #1 Panel at 11:30 AM. It was heard by the crew. Upon investigation, it was discovered that an intersection 1 block to the right of station 12711 had fallen. Dimensions- L-20', W-20', H-5'plus Support - 5' fully grouted resin; grade 60 bolts
HIGHWALL AT MOUTH OF DRIFT AT #1 ENTRY. FAN ENTRY FELL DURING IDLE SHIFT. 6' THICK EXTENDING 15' TO INBY DRIFT MOUTH. WE REBUILT CANOPY - CRIBBED ROOF FROM TOP OF CANOPY - INSTALLED HEIZEMAN HEADERS AND JACKS - RELEASED ON 4-30-04.
DRAGLINE WAS IN PROCESS OF DIGGING OFF BENCH WHEN THE BUCKET CONTACTED AND SEVERED AN ABANDONED GAS LINE. FRICTION IGNITED SOME RESIDUAL GAS TAPPED IN THE LINE. THE LINE WAS DRAINED OF ITS REMAINING SUBSTANCE (MOSTLY WATER) AND THEN REMOVED. THERE WERE NO INJURIES.
An unintentional roof fall occurred in the no.3 entry of A mains at spad 6. the fall measured 6 ft high, 18 ft wide and 22 ft long.
A roof fall in the east main #33 x-cut #9 entry measuring 50' by 20' by 6'. The area was mined in oct 2000 and supported with 6' tension rebar. The fall was caused by a hidden slip in the mine roof.
DURING NORMAL HOISTING OPERATIONS AN ELECTRICAL PROBLEM OCCURRED RESULTING IN HOISTING BEING SUSPENDED UNTIL REPAIRS WERE MADE. REPAIR REPLACEMENT OF THE FAILED ELEC. RELAY CAUSED THE HOIST TO REMAIN INOPERABLE FOR 5 HRs & 35 MIN. MSHA NOTIFICATION WAS GIVEN WITHIN 30 MINS. OF THE PROBLEM OCCURRING & IMMEDIATELY AFTER THE HOIST WAS AGAIN OPERATIONAL.

A FRICTIONAL IGNITION OCCURRED AT THE FACE OF THE #4 ENTRY WHICH HAD BEEN MINED IN 132 FEET IN THE F-12 SECTION. THE FLAMES FROM THE IGNITION LASTED APPROX ONE MINUTE AND WERE EXTINGUISHED WITH THE WASHDOWN HOSE, 1 1/4 INCH WATERLINE AND A FIRE EXTINGUISHER. THERE WERE NO INJURIES OR DAMAGE TO EUQIPMENT.
AN UNINTENTIONAL ROOF FALL OCCURRED OUTBY 4 SOUTH SECTION IN #9 ENTRY AT 90+11. THE FALL MEASURED APPROX 150' L X 16 1/2 W X 12' HIGH. NO EQUIPMENT OR PERSON INJURED.

THE AUTOMATIC ELEVATOR AT THE GARDEN B SHAFT WAS INOPERATIVE IN EXCESS OF 30 MINS. DUE TO A CIRCUIT BREAKER TRIPPING OUT ON THE INCOMING 69000 VOLT CIRCUIT.
A contractor removing siding from the tipple headframe structure inadvertantly pulled a wire out of the upper track limit switch. The mine was evacuated at 8:50 a.m. and completed at 9:08 a.m. The limit switch was repaired and tested at 9:35 a.m. Miners returned underground at 9:40 a.m.
A FRICTIONAL IGNITION OCCURRED AT THE FACE OF THE #4 ENTRY WHICH HAD BEEN MINED IN 35 FEET IN THE F-12 SECTION. THE FLAMES FROM THE IGNITION LASTED APPROXIMATELY ONE MINUTE AND WERE EXTINGUISHED WITH THE WASHDOWN HOSE AND 1 1/4 INCH WATERLINE. THERE WERE NO INJURIES OR DAMAGE TO EQUIPMENT.
A ROOF FALL CONSISTING OF LAMINATED SHALE AND SOME SMALL LAYERS OF SANDSTONE OCCURRED IN THE #4 ENTRY (BELT) OF 3RD LEFT. IT IS LOCATED 60' INBY SURVEY STATION NO 18. THE FALL MEASURES APPROXIMATELY 24' WIDE X 24' LONG X 10' HIGH. THERE WERE NO INJURIES AND NO PROPERTY DAMAGE, EXCEPT A 24' METHANE EXTENDABLE PROBE AND PROTECTOR.
A roof fall was found about 6:30 am at #27 xcut on first left in the return #1 entry. It is about 9-10' high 20' wide and about 40' long.
AN UNENTINTIONAL ROOF FALL OCCURED JUST OUTBY THE UNIT 1 WORKING SECTION IN THE INTAKE AIRCOURSE. THE FALL OCCURED AT THE +5005 FOOT MARK OF THE NO.5 ENTRY. IT MEASURED 20'X30'X5. THE FALL OCCURED DUE TO THE FACT THE ROOF STRATA BROKE AT & BELOW THE POINT OF ANCHOR. THE FALL AREA WILL NOT BE CLEANED UP.
THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES BECAUSE OF A STRETCHED GOVERNOR ROPE. THERE WERE MEN ON THE ELEVATOR AT THE TIME BUT WERE NOT ON THE ELEVATOR FOR THE MORE THAN 30 MINUTES. THE ROPE STRETCHED BECAUSE OF CHANGE IN TEMPERATURE.
ROOF FALL SPAD #2889 - #3 ENTRY, #2884 - #4 ENTRY. APPROX. 18' WIDE BY 120' LONG. WILL NOT BE CLEANED UP. DEVELOPED BETWEEN 3/1/04 AND 3/10/04 - APPROX. 1000' FROM FALL. FELL ABOVE ANCHORAGE.
The slope car was outside the slope and in the process of being jogged back to the charger and in so doing the slope car was jogged back too far causing the hoist to go into overtravel. The hoist was down 45 minutes.
#3 head drive is powered by tires. It appears the roller stopped and tires kept turning creating friction that ignited tires. Slip protection did not shut belt off.
A roof fall occurred on the mmu-004 working section @ crosscut #10 entry #5, the section is the 3rd west panell off of the 1st main south. the fall measureds approx. 30 ft long, 18 ft wide and 8 ft high. the area contained a slip with water and contained thinly caminated shale. primary support in this area is 5 ft fully grouted resin bolts.
A ROOF FALL OCCURRED MEASURING 30' LONG X 19' WIDE X 14' HIGH BEGINNING AT THE INTERSECTION OF SPAD #2423 IN #4 HEADING. THE JOY SHUTTLE CAR WAS INVOLVED AS WELL AS VENTILATION BEING DISTURBED, NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.
FIRE BROKEOUT IN CEILING OF LUNCH ROOM THE FIRE BURNT FOR AN UNSPECIFIED AMOUNT OF TIME. STRUCTURE DAMAGE ONLY.
A roof fall occurred on the 2L4W panel in entry #7 at Cross-cut 32-33. The cause of the fall was weak laminated roof strata. This entry has additional supports in it and had been dropped from the mining cycle.
A HOIST DRUM BEARING OVERHEATED AND BLEW APART IGNITING GREASE THAT IS USED TO LUBRICATE THE BEARINGS & HOIST GEARING MACHINE WAS IN OPERATION. 2-150 LB DRY CHEMICAL & 12-20 LB DRY CHEMICAL EXT. WERE USED TO PUT OUT THE FIRE. FIRE DEPT. WAS CALLED BUT NOT NEEDED. DAMAGE WAS LIMITED TO THE BEARING THAT FAILED. FIRE WAS PUT OUT IN 35-40 MINUTES.
THE SHEARER MINED INTO A HORIZONTAL DEGAS HOLE THAT HAD METHANE IN IT. THE SHEARER IGNITED THE METHANE.
A roof fall was discovered in crosscut 39 of the east mains between #1 and 2 entrys. The fall measured 25 feet long 15 feet wide and 4 feet high. 5 roof bolts were pulled out as a result of the fall and 5 bolts were rendered useless.
An unplanned roof fall occurred at spad 10538, 2900' from active face, developed 4-16-04, all entrances breakered off. The fall measured 90' length, 20' wide, 10' high. No accidents reported.
The slope hoist at the Wheatland portal malfunctioned. The hoist would not operate. After an investigation it was found that someone had hung a bundle of SO cable off the pull cord on the slope bottom. This activated an alarm in the hoist house. This required that numerous features of the hoist had to be reset.
A ROOF FALL OCCURRED IN THE BLEEDER AREA OF THE 4A SOUTHWEST PANEL. FALL IS APPROX. 18' WIDE, 25' LONG, AND 6' THICK. BLEEDER SYSTEM WAS NOT AFFECTED DUE TO MULTIPLE BLEEDER ENTRIES IN THIS AREA.
THE MINER HAD CUT APPROX. 13 FEET INTO THE LEFT SIDE OF THE #2 FACE IN FIRST LEFT SECTION. IT APPEARS THAT WHEN THE BITS ON THE MINER STRUCK AGAINST THE SANDSTONE TOP, DUST WAS IGNITED NEAR THE HEAD OF THE MINER. THE IGNITION SELF-EXTINGUISHED IMMEDIATELY.
ROOF FALL IN TAILGATE OF 10TH EAST LONGWALL - CAUSING AN IMPASSABLE CONDITION - VENTILATION WAS NOT CHANGED.
ROOF FALL OCCURRED IN AN INTERSECTION IN THE #8 ENTRY OF THE 001 SECTION. FALL IS APPROX. 26' LONG, 24' WIDE, AND 6' THICK. THE FALL IS LOCATED AT SURVEY STATION NO. 22415.

A ROOF FALL WAS DISCOVERED IN THE MAIN SOUTH #3, #2 ENTRY, INBY STATION SPAD #7388. THE FALL MEASURES APPROX. 50'LONG, 18'WIDE AND 6'HIGH. THE AREA IS SUPPORTED WITH 5'FULLY GROUTED RESIN BOLTS.
A ROOF FALL WAS FOUND ON 4/13/04 IN A 4 WAY INTERSECTION OF ENTRY 2 X-CUT 161 MAIN SOUTH. THE FALL MEASURED 30'WIDE X 30'LONG X 15'HIGH. THE AREA HAD BEEN MINED 12/15/96 AND WAS SUPPORTED WITH 8'FULL RESIN BOLTS. NO INJURY OR PROPERTY LOSS RESULTED. NW CORNER OF INTERSECTION HAD LARGE CLAY DYKE CUTTING ACROSS WHICH MAY HAVE WEAKENED THE ROOF. NO PLANS TO CLEAN UP FALL.

THIS IS A NON-INJURY WALL FAILURE. SECTION OF HIGHWALL MEASURING 120' LONG, 0' TO 12 FT THICK & APPROX 80' HIGH FELL AND INTERRUPTED THE MINING CYCLE.
BASED ON THE INFORMATION FROM THOSE PRESENT, IT APPEARED THE IGNITION WAS CAUSED WHEN THE CUTTER BITS WERE TRIMMING ROOF ROCK AND A SPARK IGNITED AN UNKNOWN METHANE BLEEDER.
A HIGHWALL FAILURE OCCURRED IN THE UNDERGROUND PIT AREA ON THE WEST SIDE OF THE HIGHWALL. VERY LITTLE ROCK WAS DISLODGED FROM THE HIGHWALL. APPROX. 14 CRACKS APPEARED IN THE SERVICE ROAD NEAR THE TOP OF THE HIGHWALL, RANGING FROM 1"" TO 21"" WIDE, FOR A DISTANCE OF 381 FEET.
NORTH MAINS MMU 004, 74 BLOCK, 5 ENTRY, 4-WAY INTERSECTION. 20 FT WIDE X 20 FT LONG X 10 FT HIGH. SUPPORT: 6 FT 3/4 INCH RESIN BOLTS, ROOF MATS, WIRE SCREEN, CCABLE TRUSS BOLTS. FALL DANGERED OFF, ADDITIONAL ROOF SUPPORT INSTALLED AROUND FALL. CLEAN UP PLAN DEVELOPED AND POSTED.
the automatic elevator at the deskins ""b"" shaft was inoperative in excess of 30 minutes while transporting supplies. the cable for the onboard controls developed an electrical fault.

FALL WAS 50'LONG BY 10' TO 20' WIDE AND ABOUT 7' TO 10' HIGH. AREA WAS BOLTED WITH 4' FULLY GROUTED RODS WITH OVERSIZE BEARING PLATES.
AN UNPLANNED ROOF FALL OCCURRED MEASURING APPROX. 125' X 24' X 20'. THE FALL OCCURRED FROM FOOTAGE MARK 1850 INBY THE FACE. THE AREA WAS INITIALLY BOLTED WITH 5&6 TORQUE TENSION BOLTS, 8""&14"" CABLE BOLTS, TRUSS BOLTS W/ 10' LEGS, AND SCREEN MESH INSTALLED W/ 4' RESIN BOLTS. NO EQUIP. INVOLVED & PASSAGE OF PERSONS NOT AFFECTED. THE FALL WILL BE CLEANED UP.

Roof Fall in 1st left section of entry #9 between spads #495 entending to spad #493 in GOB area of bleeder . Fall was approximately 150 feet long and 5 feet thick. Fall was discovered by Jerry Freeman and it was investigated by MSHA's James Duckworth. Steps taken to prevent recurrence of accident is to seal the area.

A FALL APPROX. 6 FEET HIGH, 25 FEET LONG AND 18 FEET WIDE ON THE NORTHWEST #1 BELT, #2 ENTRY, #3 X-CUT. THE FALL WAS APPROX. 4,000 FEET FROM THE ACTIVE SECTION. THE FALL WAS IN THE BELT ENTRY. ENOUGH OF THE FALL WAS REMOVED TO ALLOW ROOF BOLTING AND TO ROUTE THE BELT OVER THE FALL.
A ROOF FALL OCCURRED IN ENTRY #2 OF THE 1ST SW PANEL BETWEEN SURVEY STATIONS 7+20 & 8+40. THE FALL MEASURED 70'LONG X 18.5'WIDE X 8'THICK. THIS AREA HAD BEEN BOLTED WITH 6' & 8; FULLY GROUTED BOLTS. ALL APPROACHED TO THIS AREA HAVE BEEN TIMBERED OFF.
A ROOF FALL WAS FOUND ON 4/12/04. IT WAS DETERMINED THAT THE FALL OCCURRED ON 4/9/2004 IN THE RETURN AIR LEG OF THE WHITTAKER PORTAL FAN. THE FALL MEASURED 15.5' WIDE X 15' IN LENGTH X 6' HIGH. THE AREA WAS MINED IN JUNE OF 1976 AND WAS SUPPORTED WITH CONVENTIONAL ROOF BOLTS, POSTS, CRIBS AND PLANK.
The cedar creek service elevator was out of service from approx 7:00 pm until 9:00 pm. The elevator stopped near the top landing. The door locked and floor height were adjusted.

METHANE FRICTIONAL IGNITION - FACE AREA OF #9 ENTRY OF 6-NORTH SECTION (#070). OCCURRED ON LEFT RIB AS A RESULT OF MINER CUTTING DRUM EDGE, IGNITING A METHANE BLEEDER ON COAL RIB.
Thr roof fall occurred starting x-cut from #4 entry to #5 entry. 20' cut was taken out and then roof fell in cut and then took out intersection in #4 entry. Fall was 18' to 25' wide 6-7' high and 40' long. Area was bolted with 4' fully grouted rods with 8"" oversize bearing plates and there was a 12' cable bolt on the intersection.
Roof fall occurred in #2 entry face on 8 left panel (005 section) at #38 crosscut. Fall was approx 25' long x 18' wide x 6' thick & fell outby the pillar line & entrapped 2 mobile roof supports. 5' high tech bolts had been installed in the area, supplemented with 12' super bolts.
Wed. @ 4:02 am a rock burst occurred on 4000 level apprx 30 ft w. of 4000-197 blind raise sill in the main drift & in 3700-230 raise. (3700-230 raise is directly above area on 4000 level) Damage in 4000 drift consisted of hanging wall blowing out 10 ft of rib & back shaken up around 20 ft. Damage in 3700-230 raise consisted of back/hanging wall blowing out 6-8 ft & broken timber.

A ROOF FALL WAS FOUND IN A WORKED OUT AREA OF 5L RETURNS ON 4/6/04 AT 11 AM. FALL WAS BETWEEN #1 & #2 ENTRY AT X-CUT 11. FALL WAS 20'WIDE X 20'LONG X 10'HIGH. AREA WAS MINED NOVEMBER 15, 1998 AND HAD BEEN SUPPORTED WITH 8' FULL RESIN BOLTS. NO INJURY RESULTED BE A RETURN STOPPING WAS DESTROYED. AREA WAS PROPPED OFF AND NEW STOPPING WAS BUILT.
A roof fall occured in the #5 entry between crosscuts # 6 & 7. The fall is approx. 80 ft. long and 29 ft. wide and 4ft -7ft thick. The roof was shale and slate.
A roof fall occurred in the 11th east, #1 entry, at #145 crosscut, the fall was 20' long, 25' wide, 8' high. The fall didn't affect travel, ventilation, damage equipment, or cause any injury. The area was bolted with 6' & 8' point anchor bolts. This area is not active workings. It is an idle panel. No one was required to work or travel in this entry.

Unintentional roof fall at station #4448' up to next inby corsscut in #9 entry in the 2 er fall was inby belt tail of a working section 010-0 45' long x 18' wide x 5-6' high. 5' conventional bolts.
A ROOF FALL WAS DISCOVERED IN THE 1ST MAIN WEST #2, #5 ENTRY, CROSSCUT 35 INTERSECTION. THE FALL MEASURES APPROX. 30 FEET LONG, 6 FEET HIGH, AND 18 FEET WIDE. THE AREA WAS SUPPORTED WITH 5 FOOT FULLY GROUTED RESIN BOLTS.
Based on info received it appears the ignition was caused when the sparks from cutting torch caused a piece of slag to travel from the off-side of the machine to the coal face and ignite an unknown methane bleeder.
Hoist down in excess of 30 minutes. During severe thunderstorm, stick switch stuck in headframe. Everything cleaned and reset. Mine was evacuated. Down from 1:01 a.m. to 5:05 a.m.
#7 ENTRY OF 2 WEST-#5 ENTRY OF 2 RIGHT-THIS AREA CAVED AT 3 PM ON 4-3-04. THERE WERE NO VISIBLE SIGNS OF ANY CLETES OR CUTTERS-AREA WAS BOLTED WITH 4 BOLTS: THERE WERE NO ROOF PROBLEMS WHEN THIS AREA WAS BOLTED ON 2ND SHIFT 4-2-04.
ROCK FALL OCCURRED IN THE #1 ENTRY ON THE #1 UNIT 60 FT OUTBY THE DUMPING PT WHICH WAS BETWEEN 3 +50 & 4+50 IN THE RETURN.
A mine evacuation was initiated due to a mechanical problem with production hoist.
No.1 kiln was sht down @ 20:00 hrs on 3/31/04 for maint. repairs. Scope of work incl: replacement of thrust roller, chg F.D. fan bearing & balance, modification of burner to manufacturer spec. Work was completed 4/2/04. Maint returned kiln to operations & operations then put kiln in service. @ 20:00 a blast was heard on south end of unit.
During thunderstorm hoist down in excess of 30 minutes due to power supply breaker not resetting. Reset breaker, test runs made. Hoist down from 11:15 p.m., 4-2-04 to 2:20 a.m., 4-3-04.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE MILLENNIUM PORTAL 3N LONGWALL ENTRY #3(CROSSCUT #9). THE FALL WAS APPROX. 35'LONG X 18'WIDE AND 15'THICK. THE FALL DID NOT AFFECT VENTILATION.THE FALL WILL BE CLEANED UP.
THE SLOPE CAR WAS IN THE PROCESS OF COMING OUT OF THE MINE WHEN IT LOST POWER AND THE BRAKES SET. IT WAS APPROXIMATELY 500' FROM THE BOTTOM WITH AN EMPLOYEE IN IT. THE BATTERIES ON THE SLOPE CAR WERE CHANGED ALONG WITH AN ELECTRICAL COIL. THE CAR WAS BACK IN SEVICE AT 7:50 A.M.
AFTER FAILING TO REPORT OUT, THE VICTIM WAS DISCOVERED ON SURFACE, HAVING TRAVELED BY BELT. INVESTIGATION IS STILL ONGOING AT THIS TIME AS TO WHAT HAPPENED TO CAUSE THIS ACCIDENT.
Truck driver's lid got stuck on safety cage and when he tried to pry it free, it came loose and sprung and hit him. He fell 11 feet. This was told to me by Ralph Rodriguez.
Employee was pinned between the coal rib and the ripper head of the continuous miner as he was moving the miner from the 2 right crosscut to the 2 left crosscut by radio remote.
EMPLOYEE ALLEGES THAT WHEN HE AND TWO CO-WORKERS WERE LIFTING A SNUB ROLLER, HIS FINGER GOT STUCK IN BETWEEN THE SNUB ROLLER & THE CATWALK PULLING OFF THE TIP OF HIS LITTLE FINGER.
P-5512 was not pumping. Operator shut off pump using local start/stop station. He climbed down ladder & opened belt guard inspection door. He could not see very well because of poor lighting. He put his left hand on top of the drive belts. The pump was still turning which caused his left hand to be caught between the belt & pully & amputated the end of his left middle finger.
EE was unjamming surge box, product broke loose. Material fell from upper cleanout port thru a opening near the clean out port. Rock fell approx 10' and landed on left hand and also hitting him on lower left leg. Index finger amputated.
EE was investigating a noise coming from MC-1 unit, cleaner conveyer tail pulley. He removed the guard from the cleaner conveyer unit and inserted his right hand to check a weld on the pulley to determine if the weld was defective. The tip of the finger was caught and pinched between the bushing and the tail pulley shaft, resulting in loss of fingertip above cuticle.
WHILE RUNNING AND EMPTYING A BATTERY POWERED ROCK DUSTER, THE EMPLOYEE ACCIDENTLY PLACED HIS HAND NEAR THE DISCHARGE CATCHING HIS FINGERS.
Employee was helping check empty train cars when he somehow fell under the moving train car and damaged his left leg to the point in which it had to be amputated 6""-8"" below the knee.
EMPLOYEE LIFTED A 2550 TOOTH POINT OFF THE SHOE OF THE DRAGLINE & FELT PAIN IN HIS LOWER STOMACH AREA. BECAUSE THE PAIN DID NOT GO AWAY, EE WENT TO HIS DR. ON 4/30/04 WHERE HE WAS DIAGNOSED WITH A HERNIA. EE CONTINUED TO WORK UNTIL 5/4/04 WHEN HE WENT IN FOR SURGERY.
EE WAS ASSISTING IN SETTING STEEL ARCHES AT THE RETURN AIR SHAFT. HE WAS GUIDING THE 3 1/2 SCOOP INTO THE LEG ON THE ARCH TO INSTALL THE SPREADER PIPE AND BOLT. AS THE LEG WAS MOVED IN, HE PLACED HIS R. MIDDLE FINGER BETWEEN THE PIPE AND ARCH, RESULTING IN THE TIP OF HIS FINGER BEING AMPUTATED. EE WAS TRANSPORTED TO THE HOSPITAL AND THE FINGERTIP WAS REATTACHED.
(B) UNDERGROUND LOCATION: SECTION BELT TAIL. EE WAS ATTEMPTING TO REMOVE A CRIB TIE FROM UNDER THE FEEDER AS IT WAS BEING TRAMMED BACK ONTO THE TAIL PIECE. WHEN THE EE PLACED HIS HANDS ON THE CRIB TIE THE FEEDER TILTED BACK ONTO THE CRIBS. THE EE'S 3RD & 4TH FINGERS R. HAND WERE CAUGHT BETWEEN CRIB TIE AND MINE FLOOR - TIPS SEVERED. EE DID NOT HEAR THE ALL CLEAR SIGN.
Ee had put 1/2"" pins in tablet machine. He started it without guards in place. He thought there was a loose part & reached his rt hand into operating machine to see if it was loose. The moving machine caught his hand. He was taken to local dr, & was sent to Billings MT. Drs removed his little finger, index finger at first knuckle, & broke his middle finger.
EMPLOYEE WAS SETTING A CATHODE SAMPLE ON A RACK, IT SLIPPED OFF THE HOOK SMASHING HIS FINGER, CUTTING OFF THE TIP.
EE cut off his right middle finger while working on the hp-500 bowl. Crew was changing out the wear liners in the bowl of the HP-500. Nuts were placed under the bottom of the new liner to give the bowl clearance off the floor to allow adjustments. Grease is used to seal leaks. The grease created suction. When it released, the bowl dropped & amputated his finger.
EE WAS UNCHOKING GYPSUM FROM BASEMENT AREA. EE PICKED UP A STEEL BAR WEIGHING IN EXCESS OF 80 LBS TO BEGIN POKING AREA. AFTER 5 MINUTES, HE FELT A PAIN IN HIS STOMACH.
EE was lifting and changing out screens at the washer plant on 4-1-04 when he began to fell soreness. on 4-2-04 he loaded a belt for denver filter dishcarge in back of truck when he felt pain on right side of groin. ee went to dr on 4-5-04, 2pm
ee and changed out a wheel motor brake on a haul truck on 4/23/04 ee was diagnosed with hernia's. Due to the uncertainty of how or when the accident happened, except for the above metioned incident, this incident was not work related. This incident became reportable on 3/16/05 when this office recieved notification that the worker's compensation accepted claim.
employee placed his right arm under a suspended rockduster to align the wheels with the track and the rockduster fell on his arm.
MEMBER WAS STEPPING ACROSS TRACK AND STEPPED ON A ROOF STRAP CAUSING HIS LEFT LEG TO SLIDE OUT SIDEWAYS AND TWISTING HIS RIGHT KNEE.
EMPLOYEE WAS USING CHANNELOCKS TO LOOSEN A NUT ON A BRACKET THAT ANCHORED THE END OF THE TROLLEY WIRE. AS HE PULLED, THE CHANNELOCKS SLIPPED CAUSING HIM TO LOSE HIS BALANCE AND TWIST HIS RT. KNEE.
Employee was climbing up onto loader and slipped on the boarding ladder. He fell backwards onto the ladder fender and hit his back.
EE WAS OPERATING A CONTINUOUS MINER WHEN A ROCK MEASURING 3' WIDE 3' LONG AND 8"" THICK FELL FROM THE TOP STRIKING THE TOP OF THE CONTINUOUS MINER. THE ROCK TRAVELED ALONG THE CONVEYOR BOOM AND ROLLED OFF THE END OF THE BOOM STRIKING THE MINER OPERATOR ON THE LEFT KNEE.
EE WAS HOLDING A PIECE OF CONVEYOR BELT ATTEMPTING TO CUT IT WITH UTILITY KNIFE. AS HE PUT DOWNWARD PRESSURE ON THE KNIFE TO CUT, HE LOST HIS FOOTING IN THE GRAVEL HE WAS STANDING IN CAUSING THE KNIFE TO CUT HIS RIGHT THUMB & INDEX FINGER.
STRAIN RIGHT KNEE BECAME REPORTABLE ON 5/21/04
EMPLOYEE STATED THAT HE HURT HIS BACK HELPING TO LIFT SCREEN PANELS THAT WERE BEING INSTALLED IN THE SCREEN DECK. EMPLOYEE DID NOT REPORT PROBLEM TO MANAGEMENT UNTIL 5/3/04.
Ee was running the offside of roofbolter, they were moving the bolt machine up to the next row, when a piece of rock fell from between the ATRS and drill station, landed on his canopy and slid off, striking his right leg.
THE GROUND WAS MUDDY. EE STEPPED OFF THE LOADER AND TWISTED HIS KNEE.
EMPLOYEE STATED THAT HE WAS OPERATING A COAL HAULER TRUCK WHEN HE HIT A HOLE IN THE ROAD CAUSING HIM TO JAR HIS BACK. EE WORKED UNTIL 5/4/04, WHEN HE REPORTED THAT HIS BACK WAS SORE. HE WENT TO HIS DR. AT THAT TIME & REPORTED OFF WORK.
Employee was stepping down off of a ledge at a belt conveyer underpass. As he stepped down onto area below, he felt his knee pop. He was going to a tractor to get the blocks.
WHILE TRYING TO INSTALL A GUARD ON A PIECE OF MOBILE EQUIPMENT, THE GUEARD SLIPPED AND CONTACTED HIS HAND.
EE STATED SHE WAS TURNING OFF THE VALVE ON A 3/4"" WATER LINE WHEN SHE TURNED LOOSE THE HOSE WITH ONE HAND THE HOSE FLEW BACK HIT ABOVE RIGHT EYE
Injured party had just got out of his shuttle car when he raised up he hit a dislodged roof bolt with his neck cutting it.
EE WAS BOLTING TOP IN 6 FACE IN HIGH TOP AND BLACKED OUT TWICE - FALLING FROM WALK THRU BOLTER APPROX 6' LANDING ON HEAD & RIGHT SIDE - CUT ON TOP OF HEAD - BRUISED & STOVED RT SHOULDER, ELBOW, RT THUMB AND BRUISES ON RT SHOULDER & BACK.
EE WAS OPERATING A TRACKHOE. WHEN HE STEPPED DOWN TO WALK OVER TO CHECK HIS WORK, HE STEPPED ON A LARGE DIRT CLOD, TWISTING HIS LEFT ANKLE.
EE STATED THAT HE AND HIS WORK PARTNER WERE HANGING CABLE WHEN EE EXPERIENCED A SHARP PAIN SHOOT FROM HIS HIP AND GO DOWN HIS RIGHT LEG. EE FINISHED THIS SHIFT AND WORKED HIS NEXT REGULAR SHIFT. WENT TO THE DOCTOR ON 4/30/04. MISSED HIS FIRST SHIFT ON 5/3/04.
Employee was hanging miner cable, while moving slack, employee felt pain in his back and neck. he continued to work and twisted his left knee also.
AS EMPLOYEE WAS INSTALLING GLUE INTO HOLE FOR A GLUE BOLT A SMALL PIECE OF ROCK FELL FROM OVERHEAD CAUSING EE TO JUMP BACK STRIKING HIS RT. ELBOW ON CANOPY SLEEVE OF THE BOLTER.
EE WAS LIFTING A LARGE OXYGEN CYLINDER AT THE WEST FINLEY BUNKER TO MOVE IT TO HIS JEEP. HE FELT PAIN IN HIS ABDOMIN. HE CON'T TO WORK UNTIL HE WAS DIAGNOSED WITH A HERNIA. LOST WORK DAYS BEGAN 4/11/05
EE STATED HE WAS UNSTOPPING ROCK DUST AIR HOSE WHEN A CLOG BROKE LOOSE AND CAUSED HIM TO LOSE HIS BALANCE AND HIT HIS KNEE. EE STARTED MISSING WORK 5/19/04.
EE was working on a submersible pump pontoon in the pit. He stated that when he stepped over the pump hose, he either tripped on the hose or caught his foot toe in the expanded metal deck walkway causing him to fall on his knee. EE was wearing shorts.
WHILE INSTALLING EQUIPMENT FOR CONTRACTED AGGREGATE PROCESSING, JOB WORKERS WERE INSTALLING THE SCREEN MOTOR WITH A CRANE. AS THEY WERE ADJUSTING THE LOCATION OF THE SLING ON THE MOTOR, THE MOTOR SLIPPED AND CAUGHT THE LEG OF ONE OF THE WORKERS.
Employee had complained of low back pain for several weeks. Employee stated while preparing drill steel for drilling, he felt pain in his low back. This report has been delayed waiting on the diagnosis from the treating physician.
At the time of the incident, employee was repairing an oil leak on the rear differential of a CAT 797 off-highway truck. As he was removing/lifting the oil filter mounting block he felt a sharp pain in his lower back
EMPLOYEE WAS REPORTING TO WORK AT SCALE HOUSE. EMPLOYEE LOST FOOTING ON BANK BEHIND THE SCALE HOUSE CAUSING HER TO FALL AWKWARDLY ON LEFT ANKLE. LIGAMENTS WERE TORN IN THE LEFT ANKLE WITH SCRAPES AND ABRASIONS ON THE LEFT LEG. EMPLOYEE TRIED TO WORK UNTIL 5/4/04. WILL HAVE DAYS AWAY FROM WORK.
PER EMPLOYEE: HE WAS LEAVING FOR LUNCH & WENT TO GET CIGARETTE MONEY FROM ANOTHER EMPLOYEE & HIT HIS HEAD ON A PIECE OF ANGLE IRON ON PLANT.
EE was greasing a bridge carrier. A piece of drawrock approx 4' by 4' by 6"" fell out from between roof bolts. The rock struck the bridge carrier, breaking into two pieces. One of the pieces of rock then struck the employee.
Employee was stepping out of a track vehicle when he hyper-extended his rt knee, resulting in pain.
Laborer and an operator were removing pipe from a truck when operator closed the tailgate and pinned the laborer.
EMPLOYEE REPORTED: HE UNBOARDED RAM CAR OPERATOR'S COMPARTMENT & WAS ATTEMPTING TO PULL 2"" CONT MINER WATER HOSE OUT OF THE HAULAGE ROAD. WHEN PULLING ON THE HOSE A ""TWINGE WAS FELT TO HIS LOW BACK"". THE FOLLOWING MORNING BACK WAS STIFF & SORE. CAME TO WORK ON 4-30-04, REPORTED THE INJURY TO MINE MGT. STATED PAIN & DISCOMFORT HAD WORSENED, LEFT TO SEEK MEDICAL ATTENTION.
AT THE TIME OF THE INCIDENT, EE WAS REMOVING A DRAIN PLUG FROM THE DIFFERENTIAL OF A CAT 793 OFF-HIGHWAY TRUCK. ON 5/8/04 EE CALLED IN AND REPORTED DIFFICULTY BREATHING AND PAIN ALONG HIS RIBS; DIAGNOSIS: CRACKED RIBS.
THE EE HAD COMPLETED A PRE-SHIFT WALK AROUND ON A 240 TON HAUL TRUCK & PICKED UP A CHOCK BLOCK. THE EE THEN SWUNG THE CHOCK BLOCK IN AN UPWARD MOTION TO PLACE IT IN A CHOCK BLOCK HOLDER AND FELT A SHARP PAIN IN THE LOWER RIGHT SIDE OF BACK.
PULLED ON A STUCK DRILL STEEL AND FELT PAIN IN HIS LEFT SHOULDER.*EE CONTINUED TO WORK, HOWEVER HIS SHOULDER CONTINUED TO BOTHER HIM. HE WENT TO DR. AND TEST INDICATED A SCAPULA TEAR. WENT OFF WORK DECEMBER 22, 2004.
INJURED PARTY SQUATTED DOWN UNDERGROUND TO WORK ON A MINE PHONE & THERE WAS A PIECE OF ROCK LYING ON TOP OF THE PHONE LINE. WHEN HE MOVED THE PHONE LINE, THE ROCK FELL & HIT THE TOP OF HIS LEFT KNEE.

Employee was in #7 entry working on the miner cable by himself. He said as he was pulling on the cable trying to get it out of the conduit, he twisted his left knee.
Employee was installing ground support on the hanging wall when a rock fell and struck him. He received a laceration to his forehead. The physician did not release him to return to work.

During a 2 man procedure of changing screens on 36 Hummer, the proximal west side screen became jammed. EE positioned self on the edge of the opening to forcefully move screen. Lost balance and slipped off edge hitting left chest wall on metal edges that protrude from side of Hummer.
EE WAS CUTTING A TREE WHEN THE TREE BEGAN TO SPLIT. EE TRIED TO ESCAPE TREE BY RUNNING UP THE HILL. HE SLIPPED AND FELL CAUSING HIMSELF TO BE UNDER THE TREE AS IT CAME DOWN BREAKING HIS LEG.

Part of the loader operator's duties is to monitor material flow. While adjusting flow, the employee experienced an occupational illness, resulting in a dislocated shoulder. This occurs regularly during recreational activities and on 4/27/04 while making the above operational adjustment.

EE was working on Plant 4 screen and said he ""Bent his knee wrong"" felt like he twisted it Saturday April 24th. On April 27th an accident report was filed. EE went to see a physician from our Workmens Comp panelists who referred him to an Orthopedic specialist who recommends surgery scheduled for June 22nd 2004.
THE EMPLOYEE WAS LIFTING A 50 LB BAG THAT WAS ON THE FLOOR AND PUT IT ONTO THE SCALE. WHEN HE BENT OVER TO PICK IT UP HE USED HIS ARMS AND NOT HIS LEGS.
He was placing a metal plate on the screening plant, the plate slipped and he injured his finger.
THE EE WAS ATTEMPTING TO DISLODGE THE STEEL CABLE AND ""D"" RING FROM A SHEEVE WHEEL. THE CABLE CAME OUT OF THE SHEEVE KNOCKING HIM OVER THE RAILING INTO THE OPEN HOPPER BARGE APPROX 12 FEET. HE LANDED IN 12"" OF WATER ON A STEEL BOTTOM OF THE BARGE.

REMOVED FROM DUTIES BY DOCTOR.
HE WAS CHANGING SCREENS AND FELT SOMETHING HURTING IN HIS BACK AND SHOULDER.
WAS INSTALLING EXHAUST STACK ON OUTSIDE OF GENERATOR VAN TRAILER. WHILE REACHING FOR IMPACT GUN, EXHAUST SLIPPED AND HIS MOVEMENT TO CATCH EXHAUST MADE LADDER SLIP. HE WAS STANDING AT 6' LEVEL OF LADDER.
Employee was watching cable to insure that there was slack in cable. Supervisor heard a pop and a ""grunt"" or noise and lost power on his equipment. Supervisor discovered EE laying on ground, unconscious with with no apparent injury.
Ee reported that he was injured on 4-22-04 while working on a vessel in the prep plant. He alleges that he bent over to pick up a 4"" valve weighing about 30 lbs & injured his back & right leg. This was reported late due to the ee being at work, taking vacation & personal time before reporting the alleged injury.
Employee was fusing plastic pipe. He got his hand caught between the pipe and mashed his hand. There are no broken bones, however, employee's hand was extremely sore.
2 CRANES & WINCH TRUCK HAD TURNED HAUL TRUCK BED UPSIDE DOWN, THEN 2 CRANES LOADED THE OVERTURNED BOX ON FLATBED TRAILER. INJURED EE THEN WAS CHECKING TO SEE IF THE TRUCK BED WAS CENTERED PROPERLY ON TRAILER. A LODGED PIECE OF EARTHEN MATERIAL CAME LOOSE COMING DOWN & STRIKING EE.
AS HE WAS STARTING DOWN STAIRWAY, EMPLOYEE MIS-STEPPED AND FELL DOWN STAIRS.
Employee is stating that he felt back pain on 4-16 and 4-21 while performing duties as a deckhand on a coal barge. Employee sought medical attention on 4-21. Ee had degenerative spine disease.

WHILE OPERATING MBC EMPLOYEE TURNED TO LOOK BACK WHEN HE EXPERIENCED PAIN IN NECK.
EE WAS ASSISTING THE CREW INSTALLING CONCRETE FORMS. THE SCAFFOLDING ON WHICH THE FOREMAN WAS STANDING BECAME DISLODGED, CAUSING THE FOREMAN TO FALL. AS HE FELL, HE STRUCK EE.
Crew was drilling in location where they couldn't quite level the drill. So when making steel connections the drillers helper was having to pull on the drill steel to get it to line up so they could make up. While he was pulling on the steel the driller lowered the steel to make the connection and at that time ee felt something pop in his right elbow.
Employee was transporting bed wear strips with a fork lift. Two wear strips fell off. He placed one strip back on the fork lift. He picked the end of second strip up and twisted his back as he was trying to get it onto the fork lift.
As ee was setting the roof bolter up, a piece of drawrock fell striking his right shoulder causing injury.
Twisted left leg, while loading belt structure. he felt his leg begin to "" burn "" when he twisted to throw belt structure into scoop bucket.

EMPLOYEE AND CO-WORKERS WERE CHANGING SHEARER JACK ON MINER. AS THEY WERE PUTTING THE NEW JACK IN PLACE, AND ATTEMPTING TO LINE UP BOTTOM EYE, THE JACK MOVED AND STRUCK THE TOP OF HIS LEFT HAND CATCHING IT BETWEEN THE JACK AND MINER PLATE.
INJURED & 2 OTHER EMPLOYEES WERE DISMOUNTING AN 1800 X 33 TIRE FROM FRONT AXLE OF TRUCK. HE INSTALLED HYDRAULIC BEAD BREAKER ON THE TRUCK SIDE OF THE WHEEL TO PUSH TIRE OFF OF RIM. WHEN PRESSURE WAS APPLIED TO BREAKER IT CAME OFF THE WHEEL. HIS HAND WAS ON THE BREAKER WHEN IT CAME LOOSE AND WAS MOMENTARILY STRUCK BETWEEN THE TRUCK FRAME AND THE BREAKER.
WAS WORKING ON DRY SCREEN. WAS HITTING A PUNCH TO LOOSEN A SIDE NAIL USING A 3 LB HAMMER. MISSED THE PUNCH & HIT SMALL FINGER ON LEFT HAND.
Employee was exiting off of grader, when he felt a pain in his back.
EE WAS OPERATING A SCOOP IN THE #6 ENTRY 4 BREAKS OUTBY THE SECTION AND RAN OVER A CRIB BLOCK. HE WAS BOUNCED IN THE DECK OF THE SCOOP CAUSING HIM TO HIT HIS HEAD OVER THE SCOOP CANOPY & HIS BACK ON THE SCOOP FRAME OF THE DECK CAUSING INJURIES.
Alleged accident was to have occurred while this employee was laying track. He stated that after being bent over most of the day when he straightened up he felt his back get stiff. Cause of the injury was lifting techniques. Compliance with rules/regs not a factor.
WHILE TRYING TO TURN THE NEW GOB CHUTE THAT HE WAS FABRICATING, EE WAS USING A STEERING LOADER TO PICK IT UP & USE A PRY BAR TO TURN IT OVER. AS HE WAS TURNING IT OVER, THE CHUTE SLIPPED CAUSING THE BAR TO PIN AGAINST THE FLOOR, WITH HIS FOOT IN BETWEEN.

THE INJURED WAS PLUGGING UP CHARGER TO A DEAD SET OF RAM BATTERIES, WHEN HE DID THIS THE PLUG BLEW UP & BURNT HIS RIGHT HAND. THE CHARGER WAS NOT TURNED OFF AND IT WAS POSSIBLE THAT THE 2 LEADS HAD GOTTEN TOGETHER IN PLUG.
Pit truck driver went to get out of the truck and his back felt stiff. The next morning he called in because he could not get out of bed. He went to the dr. and was told he had a pinched nerve in his lower back and would be off work the rest of the week.
EMPLOYEE STATES HE STRAINED HIS BACK WHILE LIFTING 30"" TUBES. HE WAS OFF WORK 3 DAYS AND RECEIVED PHYSICAL THERAPY.

EE WAS MIXING M-PB20-ACC WHEN AN IRRITATION AROUND HIS WRIST BEGAN. THE VELCRO GLOVE HE WAS WEARING WAS TIGHT AROUND HIS WRIST. THE POWDER & HIS SWEAT CAUSED A BURN AREA AROUND HIS WRIST.

REPAIRING DRIVE SHAFT IN A 988 LOADER, STRAINED LOWER BACK.
Employee stated that he was in process of tightening roller cap bolts on a D-10 bulldozer. During the process, he inadvertently struck the trigger causing the impact wrench to thrust his right hand against the roller. He sustained a bruise and later on, was told that he may have a torn ligament in his knuckle area. Note: Employee did not start missing work until May 5, 2004.

EE alleges back pain after turning around to get off work stool in shipping silo operator's booth. EE was offered medical treatment after reporting the condition, but refused until she requested medical attention with her personal physician on 5/2/04. She received medical evaluation on 5/3/04 and began loosing time from work on that date.
Ee injured his right shoulder while busting a rock (2' x 2 1/2' x 3"") which was fouled in the feeder.
EMPLOYEE WAS STEPPING OVER A RAISED TRACK RAIL. AS HE WAS STEPPING OVER THE RAIL THE RAIL FELL OFF THE TWO TRACK JACKS HOLDING IT UP IN THE AIR THUS STRIKING HIS RIGHT LOWER LEG. CAUSE OF THIS INJURY WAS NOT SECURING RAISED TRACK RAIL.
Employee injured back while using Coal Hauler to bump Continuous Miner away from rib, so it could be repaired.

EMPLOYEE BENT OVER TO PICK UP A DOZER TRUNNION BALL FROM THE FLOOR. WHILE LIFTING BALL ONTO SHOP TABLE HE STRAINED LOWER BACK MUSCLES. WAS UNABLE TO RETURN FOR REGULAR WORK SCHEDULE ON 4/25/04.

Excavator operator was cleaning along the high wall when a large piece of rock was dislodged and rolled into the cab pinning the operator in the cab for a period of 2 hrs and 47 minutes. Victim suffered a serious injury to the neck.
EE STATED HE TRIPPED OVER SOMETHING AND HURT HIS LEFT LEG. THE INVESTGATION REVEALED NOTHING LYING ON FOOTWALL.
EMPLOYEE WAS MOVING MINER CABLE WHEN HE TWISTED HIS RIGHT KNEE.

WHILE PUTTING OIL IN BULL GEAR BEARING, EE SLIPPED AND FELL AGAINST HOIST DRUM. EE WORKED FROM 4/19/04 UP TO 9/22/04 BEFORE HE STARTED LOSING TIME ON HIS SHOULDER INJURY.
WHILE WORKING ON 7A SECTION, EMPLOYEE EXPERIENCED PAIN IN LOWER BACK. EMPLOYEE HAS PREVIOUS HISTORY OF BACK PROBLEMS, WITH A DIAGNOSIS OF DEGENERATIVE DISEASE. RECEIVED REPORT ON 4-23-04.
Employee was attempting to fill the hydraulic tank on a D11RCD. As the tank cap was too tight to remove by hand, employee attempted to remove the cap with a 12"" crescent wrench via the cap prongs. While pushing on the cap prongs with the wrench, the prongs broke, causing emplyee to loose balance and fall to the ground, approximately 11'.
Mechanic was standing on diagonal brace of d10 Dozer. The ee's foot slipped off of bace and he fell between brace and main frame.
Their were high winds the day of the injury. The employee was working on the floor when a high wind blew lime dust into his eyes and face. The employee had safety glasses on at the time of the injury. The employee had his eye and face flushed immediately. The employee received medical treatment, Rx eye drops, and missed two days of work due to his injury.
Victim was under a dozer knocking out a pin, when something popped in his neck.
WHILE LETTING THE DRILL MAST DOWN, A ROOF BOLTER OPERATOR CAUGHT HIS LEFT LITTLE FINGER BETWEEN THE DRILL MAST AND THE SLIDE FRACTURING IT.
EE was roof bolting when a piece of rock 2'x2'x2"" fell out between the roof bolts, striking the ee on the left shoulder and back area.
Operator was using loader to clean coal in the pit. The bucket on the loader rolled down and caught hard section of rock causing the loader to jerk to a stop. Pulled muscle in his neck.

The millwright was removing screen cloth from 402 screen in the mill bilding. as he pulled the screen cloth, it dropped and pinched his left hand between the cloth and screen side.

EMPLOYEE ALLEDGE THAT WHILE IN THE PROCESS OF REMOVING A METAL PIECE STUCK AT THE DISCHARGE OF THE RECLAIMER TO THE CONVEYOR BELT # 13, HE FELT A SUDDEN PAIN IN HIS BACK AND LOST HIS BALANCE. THE EMPLOYEE CONTINUED WORKING HIS REGULAR SHIFTS. ON 4/19/04 EMPLOYEE REQUESTED MEDICAL ATTENTION FOR HIS BACK PAINS. ON 4/21/04 DOCTORS RECOMMENDED BED REST FOR 7 DAYS.
Employee was injured on 4-15-04 by a rock which he was lifting that broke in half and fell against his left shin/ ankle. The ee felt that the injury was incidental until 4-20-04 when his ankle swelled up in a knot and the skin broke for drainage. We sent him to the medical center to be examined.
Employee was unloading a deck bushing for a drill from a pickup truck to the mechanics drop when he dropped the part. When he bent over to pick up part he felt a pain in his lower back.
During stone splitting operatin minor cut left thumb on sharp piece of stone. guaze pad and tape to control bleeding were used. minor was transported to potean hospital and received stitches.

Employee's right knee was bruised while he was kneeling down working on a Caterpillar 773D quarry truck. Employee did not seek medical attention until 19 April 2004. He missed one day of work.
EE WAS OUTBY ASSISTING A TRUSS BOLTER BY THREADING TOGETHER 2 SECTIONS OF A BOLT, BY HAND AND HIS KNEE GAVE OUT. HE RESTED A FEW MINUTES AND WAS ATTEMPTING TO PICK UP A 6"" X 6"" TIMBER AND HIS KNEE GAVE OUT AGAIN.
NEAR THE END OF THE SHIFT, EE WAS PUTTING HIS TOOLS AWAY BESIDE THE PARTS CAR. STONE AND COAL FELL FROM THE MINE RIB, HITTING HIS LEGS AGAINST THE METAL RAILING AT THE BOTTOM OF THE PARTS CAR.
EE WAS OPERATING A 793 ROCK TRUCK. SHOVEL OPERATOR DROPPED A ROCK IN THE BED OF THE TRUCK AND IT SHIFTED EE LEFT TO RIGHT HURTING HIS LOWER BACK.

HE WAS ON HIS LEFT KNEE AND CLEANING OUT A SPOT TO PLACE A TIMBER WHEN A PIECE OF ROCK APPROX. 33""X 35""X 2""THICK FELL FROM BETWEEN BOLTS AND STRUCK HIM ON THE LEFT LEG, ABOVE THE ANKLE.
EE SAID HE BACKED OVER A ROCK WHILE TRACKING A SLOPE.
EE HAD FINISHED GREASING FUEL FILTERS OF DRILL.TURNED TO CLIMB DOWN STEPS, SLIPPED & FELL OFF THE DRILL PLATFORM. STRUCK GROUND WITH LEFT FOOT BREAKING LOWER LEFT LEG (BETWEEN KNEE & ANKLE)
While pushing a wheelbarrow, employee twisted his right knee.
THE DRILL OPERATOR HAD LOOSENED THE ELECTRICAL PANEL ON DRILL AND PROPPED PANEL ON A CAN OF ETHER. EE WAS INSTALLING A CB RADIO IN DRILL WHERE THE CONNECTIONS WERE BEHIND THIS PANEL. HE APPARENTLY BUMPED THE PANEL AND THE CAN ROLLED AGAINST THE WIRES CAUSING THE CAN TO EXPLODE AND SETTING EE ON FIRE. EE WAS BURNED ON BOTH THE RIGHT AND LEFT INNER THIGHS.
Primary stalled out and had to be dug out. Locking the machine out and beginning to dig the crusher out. Employee dropped a rock which landed on the left foot of injured. Left foot is swollen and tender. Ex-rays taken/no fractures.ice pack, foot elevated. Ortho shoe,crutches given. 5 days off- no weight on foot.
A BLOCK OF COAL HAD FALLEN BETWEEN BELTS WHILE PRODUCING. THE EE REACHED IN TO REMOVE THE BLOCK OF COAL WHILE THE BELT WAS STILL RUNNING, CATCHING HIS ARM IN THE BELT STRUCTURE.
EMPLOYEE WAS CHANGING SCREENS ON A CONE CRUSHER. AS HE THREW AN OLD SCREEN OFF THE SIDE, IT SNAGGED THE ZIPPER PULL ON HIS VEST AND PULLED HIM OFF THE SIDE OF THE DECK. HE WAS NOT WEARING HIS FALL PROTECTION. HE FELL APPROXIMATELY 13' TO THE GROUND. HE WAS DIAGNOSED WITH A BROKEN LEFT ARM AND A RUPTURED SPLEEN. HE WILL MISS 10-20 DAYS THEN RETURN WITH ""NO LIFTING"" RESTRICTION.

THE OPERATOR OF AN H-65 DEMAG EXCAVATOR WAS BEING DIRECTED INTO A MECHANIC'S SHED WHEN HE TRIED TO GET THE ATTENTION OF THE PERSON DIRECTING HIM, THE CONTRACTOR EE. THE OPERATOR LEANED OUT OF THE OPERATOR'S CAB TO TALK TO EE WHEN THE OPERATOR'S SWING CONTROL LEVER WAS ACCIDENTALLY BUMPED, CAUSING THE MACHINE TO SWING, HITTING EE ON THE LEG.
DID NOT START LOSING TIME UNTIL 5/17/04. While lifting block, strained neck and arm, hand, and fingers started hurting.
The employee was assisting his partner with bolting the rib when a rock fell from the rib and struck his right foot. The metatarsals for toes 2 - 5 were fractured.
Unknown - employee doesn't know of a specific incident that his pains began. He did say he was just walking and his knee began to hurt. In his opinion his problem is a result of shoveling the saw cuts.
A ROLLER BEARING WENT DOWN IN THE OUTER PLANETARY CAP OF THE JOY 14CMS CONT MINER. EE WAS HELPING TO CHANGE THE BEARING WHEN THE BEARING CAP FELL OUT AND SMASHED HIS RIGHT HAND BETWEEN THE CAP & A CRIB BLOCK.
Employee was operating headgate drum of longwall shearer when a large piece of shale (approximately 2' x 2' x 10.5') fell from roof above coal seam onto shearer body and into walkway, striking employee below the waist. Employee was diagnosed with fractures to left femur, right tibia/fibula and right ankle, and soft tissue injury to left knee.
EMPLOYEE WAS USING A COME-ALING TO MOVE THE SHEAR CUTTER MOTOR OFF THE FACE. HE COMPLAINED OF PAIN IN HIS GROIN.
Another employee was replacing mixer chutes on Mixer #518. Job scope required him to pound a chute bolt out. While pounding the bolt from the chute, the bolt flew out and struck injured employee in lower abdominal area. First aid was performed on the job site.

HE INFORMED THE SUPERVISOR THAT HE FELT IT WAS A BASIC SORE MUSCLE AND HE DID NOT WANT MEDICAL ATTENTION. SELF-TREATMENT WITH HOME MEASURES. IT PROGREESS TO VERY PAINFUL SO THAT IT EFFECTED HIS SLEEP. HE REPORTED WORSENED CONDITION ON NIGHT SHIFT 4/19.
Employee was reaching overhead to unhook bags. The left side of the middle of his back began to feel tight. (like a muscle spasm)
Driver Said truck jumped out of grear going down bottom steep truck would not stop. asked about braces owner said they had put new brakes on recently.

HE PLANTED HIS LEFT FOOT & WHEN HE RAISED HIS RIGHT FOOT HIS KNEE BUCKLED RESULTING IN A STRAIN. EE HAS BEEN OFF WORK SINCE ELECTING TO TAKE A VACATION. Dr. SAID HE COULD RETURN 5/2/04 UNRESTRICTED.

THE INJURED HAD LOST POWER TO MINER (MACHINE) DUE TO THE CABLE BEING STRUCK BY RAM CAR. THERE WAS NO VISIBLE DAMAGE. WHEN POWER WAS PUT BACK UP ON MINER BREAKER, THE CABLE BLEW UP, CAUSING A FLASH. THE INJURED WAS LOOKING AT THE CABLE WHEN IT BLEW UP. HE RECEIVED FLASH BURN TO EYES.
Employee was helping repair the d-10r dozer when he either wiped his eye and got piece of metal in his eye or the wind blew the metal into his eye.
SHUTTLE CAR OPERATOR WAS LEAVING THE MINER WITH HIS BUGGY LOADED WITH COAL. AS HE MADE A LEFT TURN, THE WHEEL UNIT ON THE DUMPING END, REEL SIDE OF SHUTTLE CAR BROKE OFF. THE BUGGY STOPPED ABRUPTLY CAUSING THE OPERATOR TO PITCH FORWARD & TO THE LEFT. THIS CAUSED HIM TO STRIKE THE LEFT SIDE OF HIS HEAD INTO THE CONTROL AREA OF THE SHUTTLE CAR.
TIGHTENING TANDEM ON PASSENGER SIDE. USING LUG WRENCH TO TIGHTEN LUGS, EMPLOYEE WAS PRESSING DOWN WITH FOOT. SAID THAT HIS FOOT WAS SLIPPING OFF SO HE SWITCHED TO THE OTHER SIDE AND BEGAN TO PULL ON LUG WRENCH STRAINING HIS BACK.
SPLICING BLET WHEN A BELT NAIL HEAD FLEW UP AND HIT HIM IN THE LEFT EYE.
EE WAS HELPING TO CLEAN OUT THE CHUTE TO THE IMPACT CRUSHER. HE WAS MOVING THE JACKHAMMER AND MASHED HIS LITTLE FINGER BETWEEN THE JACKHAMMER AND THE SIDE OF THE IMPACT CRUSHER.
EE WAS DRIVING A JOHNSON STINGER WHEN THE STEERING WHEEL SPUN AROUND HITTING EE'S RIGHT HAND CAUSING A BROKEN BONE.
EE WAS CLEANING THE APRON OF THE DUMP TRUCK WITH THE BED RAISED WHEN SOME WET LIME MATERIAL FROM THE BULKHEAD FELL, STRIKING HIS LEFT SIDE.
Was driving a mantrip out of mine when the front tire struck a rock or rut wipping steering wheel striking his hand.
EE WAS PULLING 6"" WATER PUMP HOSE, AND PULLED MUSCLE IN LOWER BACK. CONTINUED TO WORK UP TO 4-26-04 TAKING THERAPY. DR ADVISED TO TAKE OFF 4 OR 5 DAYS TO SEE IF IT HELPED BACK.
Employee was on the back of a truck. He stepped on the tailgate and it gave way causing him to fall.
EE WAS SHOVELING AROUND STACKER TAIL ROLLER, WHEN A LOOSE PIECE OF ROCK BOUNCED OFF BELT STRIKING EE ON THE LEFT SIDE OF THE FACE AND FOREHEAD.
The employee was climbing a ladder so that we could finish a welding project on a coal truck, when the bottom of the ladder slipped. The employee fell to the floor landing on his back and buttocks. The employee was treated at the minesite and taken to the hospital for an evaluation.
Employee was riding on a diesel mantrip when he hit a hole in the roadway. His back started hurting from the impact. He went to the Dr and x-rays were negative. He went back to the Dr and started therapy on 4-26-04.
WAS HELPING WORK ON A 215C A JOY SHUTTLE CAR. REEL CHAIN CAME OFF SPROCKET & HUNG UP CHAIN. WAS PRYING CHAIN BACK ON WITH SLATE BAR AND PULLED MUSCLE IN BACK.
Employee had been standing on the #5 main belt tailpiece. when he stepped off, he slipped and fell injuring his left hip/thigh area.
Employee was carrying a stone in the quarry when he slipped on mud. EE dropped the stone, stone hit his leg, causing contusion and abrasion of the left thigh.

EE SLIPPED ON CROSS-OVER WHILE WALKING THROUGH THE WALK-THRU AND TURNED LEFT ANKLE, CAUSING A HAIRLINE FRACTURE.

EE was walking in his work area when he stepped on a piece of scale, twisting his left knee. Eventually had surgery on 11/10/2004.
WHILE WORKING ON WATER LINES FOR SCRUBBER, EMPLOYEE STEPPED WRONG ON A PIECE OF SHALE CAUSING HIS KNEE TO BEND IN THE WRONG DIRECTION WHICH IN TURN CAUSED A DISLOCATION.
Shoveling in order to clean up spill of material. Prior physical condition may have been a factor. The job that was performed was routine and part of regular job.
EMPLOYEE WAS CRUSHED BETWEEN A COAL RIB AND A BATTERY POWERED ROCKDUSTER BEING PULLED BY A BATTERY POWERED SCOOP RESULTING IN A BROKEN LEFT SHOULDER BLADE AND BROKEN LEFT ARM. HE ALSO RECEIVED MULTIPLE CONTUSIONS.
EMPLOYEE WAS TAKING OFF NUTS TO CHANGE JAW PLATE AROUND JAW CRUSHER. THE EMPLOYEE WAS USING A 5-N-1 TOOL TO LOOSEN NUTS WITH. EMPLOYEE TAPPED THE PIN ON THE 5-N-1 TOOL WITH A HAMMER TO RELEASE IT AND WHEN HE DID THE HANDLE SPUN AROUND HITTING EMPLOYEE ON TOP OF RIGHT HAND.
EE WAS REMOVING CRUSHER BOWL WHEN HE GOT PINNED IN TUGGER CABLE
EE STEPPED ON A NAIL THAT WAS DROVE THROUGH A BIT BUCKET LID. PUNCTURE WOUND TO LEFT FOOT. EE WENT TO DR AFTER HE LEFT WORK. OFF WORK 4 DAYS. RETURN 4-16-04 FULL DUTY.
THE EE STATED HE WAS REPLACING BOTTOM ROLLERS ON THE SECTION BELT INSIDE THE TAKE UP WHEN HE STEPPED ON THE CROSS MEMBER HE SLIPPED & TWISTED HIS RIGHT KNEE.
EE WAS LIFTING THE BELT OFF OF THE SNUB ROLLER WITH A SET OF COMEALONGS HOOKED TO A PIPE THAT HAD BEEN PLACED UNDER THE BELT. THE EYELET IN THE PIPE THAT THE COMEALONGS WERE HOOKED INTO FAILED, ALLOWING THE PIPE TO KICK OUT & STRIKE EE ON THE ARM.
EE WAS SHIFTING ELECTRICAL LOAD FROM ONE BREAKER TO ANOTHER. BREAKERS WERE THROWN, LINES WERE CHECKED FOR RESIDUAL LOAD. WIRES WERE MOVED AND EE WAS TIGHTENING STUDS. THE LINE OR BREAKER REENERGIZED AND SPARKED, BURNING EE.
The slack in the longwall belt monorail trolleys was being pulled up. A sheave wheel and wire rope was being used. The injured employee walked along he belt placing himself in the ""vee"" of the wire rope. The sheave wheel came loose from it's mounting and flew across the belt striking the injured employee in the jaw. This resulted in a broken jaw.
EE WAS HELPING TO MOVE MINER FROM #3 HEADING TO #2. HE WAS LAYING CABLE UP ON SIDE OF MINER AND THE OPERATOR LET HEAD DOWN ONTO EE'S HAND. THIS BROKE EE'S THUMB ON RIGHT HAND.
OPERATOR WAS REMOVING SERVICE DOOR ON LOADER WHEN HE LOST HIS FOOTING & FELL 5' TO THE GROUND, WHERE DIRECT FORCE TRAUMA RESULTED IN BOTH WRISTS BEING FRACTURED. WORK AREA HAD BEEN WET DOWN FOR DUST CONTROL PURPOSES & WAS MUDDY AS A RESULT OF SUCH ACTION.
On April 09, 2004. the employee was climbing a ladder while carrying a 1000 watt electrical lamp. He slipped on the rung of a ladder and caught himself with his right arm. He reported pain in his right shoulder. At the time no medical treatment was sought. On June 04, 2004 the employee reported that he has been receiving physical therapy. To date no time has been lost.
Victim grinding conduit coupling in rack to modify conduit, hand grinder kicked out and struck victim on left forearm, causing a laceration, requiring emergency room procedures.

EMPLOYEE WAS PALLETIZING MATERAIL (CAPSTONES) WHEN A PIECE TIPPED OVER & PINCHED HIS LEG AGAINST THE PALLET. EE REPORTED BRUISING & SOME SWELLING BUT CONTINUED TO WORK. AFTER 2 WEEKS HE WAS COMPLAINING OF PAIN & DECIDED TO GO TO THE DOCTOR. DR DID X-RAYS & FOUND A FRACTURE IN THE TIBIA. 1ST VISIT TO DR ON 4-23-04. EE WORKED FROM 4-9-04 TO 4-23-04.
WHILE WELDING ON A TRUCK EXHAUST HAD TO KNEE ON THE FRAME. A PIECE OF HOT METAL ROLLED DOWN AND BURNED HIS KNEE.
Two employees were in a manlift at the construction site. The lift was inadvertently raised into a beam. One employee was caught between the manbasket and a metal frame. His chin was cut and a vertabra in his neck/back was injured. He is losing time.
Employee was carrying a piece of channel iron when he lost his footing and slipped causing lower back pain.

EMPLOYEE WAS BUILDING A CRIB, LIFTED A CRIB BLOCK OVER HIS HEAD AND PULLED A MUSCLE IN HIS NECK.
EE was pulling rope through conduit. EE had arm extended and rope caught on bolt and EE arm was overextended.
EMPLOYEE WAS STRUCK BY A ROCK THAT FELL FROM BETWEEN THE BOLTS, WHEN HE FELT PAIN IN HIS LOWER BACK.
Victim was struck in the back of the head by a hose that blew loose on the Anfo truck. He complained of being dizzy for a little while, then felt better and returned to work. On 6/18/04 he again complained of being dizzy and was sent to the doctor. He has been off since that date is in physical therapy.
Accidently hit down lever on feeder resulting in injury to left foot.
TWO EE'S WERE WORKING ON SCREENING PLANT TO REPLACE THE TEFLON SHEET ON THE RIP RAP OUTLET. AFTER REPLACING PART, EE HAD HIS HAND PROPPED ON BRACE. CO-WORKER CRANKED UP PLANT. WHEN IT WENT INTO MOTION, A ROCK THAT WAS ON CONVEYOR BELT BOUNCED OFF HITTING EE'S LEFT HAND MIDDLE FINGER. ALL OPERATION CAME TO A HALT WHILE EE WAS TRANSFERRED TO THE HOSPITAL.
MAN WAS BACKING AROUND CORNER WHEN HE CAUGHT HIS ANKLE BETWEEN CORNER & DECK OF CUTTING MACHINE.
Employee was pulling 60"" bottom roller under belt through mud when rope slipped off and he fell on his left shoulder and knee. On 11/2/05 employee had surgery to repair his left knee and began missing work at that time. Shoulder surgery is scheduled for 11/23/05.
While the employee was cleaning the plant structure with a hot water cleaning system (steam genie), the wand of the system was broken and the hot water burned the employee on the left forearm.

Truck driver backed under end loader with bucket raised and rock fell from bucket and hit in empty bed of truck.
ROCK FELL BETWEEN RIB AND BOLTS HITTING EE ON BACK, KNOCKING HIM DOWN. HE FELL ON HIS SRU.
EMPLOYEE STATED HE WAS OPENING INBY AIRLOCK DOOR GOING TO MOTOR PIT WHEN THE DOOR HUNG ON GRAVEL CAUSING HIM TO BE CAUGHT OFF BALANCE TWISTING HIS RT. KNEE. HIS FIRST LOST WORK DAY DID NOT OCCUR UNTIL 5/11/04 DUE TO SURGERY.
UNLOADING A CACHE BOX OF SCSR'S FROM THE FORKLIFT SCOOP. THE BOX SLID & HE CAUGHT HIS RIGHT INDEX FINGER BETWEEN THE FORK AND THE BOX LACERATING THE FINGER TIP.
While driving to get reloaded with sand he hit a pot hole, which caused him to bounce around causing him to strain his lower neck and upper back.
Employee was standing on top of an enclosed rail hopper car straddling the top hatch door. He stopped over door and lifted it open with both hands injuring his right elbow.
EE got out of a pickup truck and set the parking brake. When he exited the truck, the parking brake failed. The truck rolled backward and ran over EE'S leg. His leg was bruised. He did not seek medical attention until 12 April 2004 at which time it was discovered that his leg was fractured. EE is losing time as of 12 April 2004.
EE MOVING TRACK HOE IN QUARRY. PARKED HOE. GETTING OUT, SLIPPED OFF TRACK. FELL ON RIGHT SIDE.
Operator climbed into dozer to start work shift, got back out to clean the windows and fell backwards onto the tracks and then to the ground. Bruising to this left hip and shoulder. Small laceration on left hand. Small laceration on left hip which required one stitch to close.
EE states reaching into cab fdr, cb other backstead to grab handle and missed fell and landed on my back. the ee was examined x ray any provide to have and discerable injury. ee refeaered for follow up and failed to provide us with any documention sending it as a lost time injury. we continued to question the validity of the incident.
EE WAS MOVING 3"" PUMP AT THE HIGHWALL MINER/YOAST PIT, WHEN A LOOSE PIECE OF COAL SLID FROM UNDERFOOT, CAUSING EMPLOYEE TO TWIST HIS RIGHT KNEE. A REPORT WAS MADE, BUT MEDICAL ATTENTION WASN'T NEEDED UNTIL 4/26, WHEN EE RE-TWISTED THE SAME KNEE. EE HAD MRI ON 4/27, BUT THE DR COULDN'T READ IT UNTIL 4/28. EE SCHEDULED SURGERY ON 4/29. DIDN'T BECOME REPORTABLE UNTIL 4/20/04.
5H section, belt move, carrying belt roller and slipped while walking and twisted left knee. Continued working normal job duties until lost work days began 5-11-04.
EE WAS USING A SLATE BAR TO REMOVE A PIECE OF ROCK FROM OFF OF THE CONTINUOUS MINER WHEN HE INJURED HIS BACK. THE EE HAS CONTINUED TO WORK UNTIL 4/23/04 AT WHICH TIME HE BEGAN LOSING TIME.LAST DAY WORKED, 4/23/04.
Wind blowing into eyes although he was wearing safety glasses metal flew in his left eye.
Utilityman positioned a large, heavy pipe, which was rigged with a lifting device. Utilityman lifted the pipe on one end to get it to roll on a roller stand so he could get the pipe in position.
Ee was checking center line when a rock fell from overhead striking him on the head and shoulder. He went to the local emergency room where he was examined, x-rayed and given a full return to work release. Ee continued to experience pain and became a lost time as of July 13, 2004.
EE WAS CARRYING SPLICING BAR IN ON 4 EASTBELT, FELT PAIN IN RIGHT SHOULDER.
EE HAD GOTTEN OUT OF HIS PIT TRUCK (769D) TO MOVE A ROCK AFTER HE HAD DUMPED IN THE JAW. HE THEN WENT TO CLIMB BACK INTO THE TRUCK WHEN HE MISSED THE HADNDRAIL AND SLIPPED. HE FELL BACK AND LANDED ON HIS LEFT SIDE. HE WAS COMPLAINING ABOUT PAIN IN HIS LEFT LEG AROUND HIS THIGH. HE WAS THEN TRANSPORTED BY AMBULANCE TO UNC E.R.
EE TOOK GUARD OFF TO SCRAPE RETURN ROLLER WITH SHOVEL. THE RETURN ROLLER GRABBED THE SHOVEL AND PULLED EE'S HAND AND FOREARM INTO ROLLER. HE HAD MULTIPLE ABRASIONS ON LEFT FOREARM AND CUT ON FINGERTIP. ANOTHER EE REPORTED ACCIDENT.
Tramming empty shuttle car over uneven bottom back to continuous miner. Shuttle car bounced casuing the seat back to come out of back hinges. Injured fell forward striking the panic bar with his left shoulder. (Injured did not request medical treatment until 04/26/2004.)
Employee was removing a hose clamp from an oil level sight glass on a 120 excavator when the screwdriver slipped and punctured his left hand.
Injured was assisting in supplying the roof bolt machine. While he was located behind it, the operator started the machine. The cable reel had been left in the on position. As the cable quickly reeled in, it took EE's feet from beneath him. He landed on his knees causing him to twist his right knee.
Employee was stacking bags of lime and twisted lower back, which caused a strain. He is currently being treated by a chiropractor daily.
Surveying a 90 degree diamond drill hole on the drill platform. Employee stepped on a deck plank next to the drill opening and slipped through the platform. Landed below about 3 feet. Hard impact to foot caused bruising and swelling. First shift missed was 4/5.
INJURED STATED THAT AS HE WAS ATTEMPTING TO PUT A SHOVELFUL OF COAL ON THE BELT, HE FELT/HEARD A POP IN HIS LOWER STOMACH AREA. AFTERWARDS A KNOT APPEARED. INJURED WAS TAKEN TO THE HOSPITAL AND HAS A BROKEN PELVIS.
EE STARTED ROCK TRUCK TO WARM UP. HE CAME BACK DOWN LADDER AND CAUGHT FOOT IN LAST RUNG AS HE WAS STEPPING OFF LADDER. TURNED KNEE. EE WORKED THE DAY OF THE INJURY AND THE DAY AFTER. HE MISSED THE THIRD DAY TO DO TO DOCTOR.
Employee was standing on an angled support with the right knee braced against a concrete peir at ground level to tighten a fan bearing bolt. His left foot slipped on the angle, shifting stress to his right knee
EMPLOYEE HAD GONE TO CHECK THE BELT TAIL ROLLER AREA OF THE CONVEYOR BELT TO SEE WHY THE BELT WOULD NOT START. AS HE TURNED TO WALK AWAY FROM THE AREA HE STRUCK HIS HEAD ON METAL STRUCTURE. NOTE: EMPLOYEE'S FIRST DAY OFF WORK DUE TO INCIDENT WAS 4-7-04.
EE WAS INSTALLING ROOF BOLTS. WHILE PUTTING HEAD OF BOLT IN DRILL CHUCK TO PUSH BOLT UP AND TIGHTEN, HE SPUN BOLT CATCHING HIS GLOVE ON PLATE AND JERKING HIS RIGHT ARM.
Employee was setting timbers and a rib roll came out knocking one of the timbers striking employee on left leg.
Employee was hit in the foot above his steel toe by a steel impact plate.
EE WAS SUPPLYING #3 ROOF BOLTER. HE WAS LOADING RESIN GLUE ON BACK OF THE BOLTER AND WHEN HE THREW A BOX OF GLUE HE TWISTED A LITTLE & PULLED MUSCLE IN HIS LOWER BACK.
A trackman claimed that he was on his knees using a wrench and when he tried to stand up his back ""locked up"" and he was unable to stand.
EE was standing on the hydraulic tank of 98 DML overburden drill, working on an air hose. After completing the work he began to climb down from the tank. He caught his foot on the climbing surface, lost his balance and fell 3-4 ft to the deck of the drill, bruising his left shin and left elbow.
EE WAS SCALING A LOAD OF COAL AT BRANCH SCALES. HE WAS STANDING ON DRIVERS SIDE FUEL TANK STEP LOST HIS FOOTING AND FELL FROM STEP INTO MUD & WATER BESIDE THE SCALE.
While dozer was being serviced by greasers, the mechanic began doing maintenance on the track rollers. The fuel cap rolled off the machine and hit him in the head. Required fifteen (15) clips/staples to close.
Ee was on a catwalk, had no harness, fell off the catwalk and fell 20 ft.
EMPLOYEE STATED HE WAS PULLING ON CONVEYOR BELT IN ORDER TO PUT BELT BACK TOGETHER AFTER SPLICING AND FELT A PAIN IN HIS LEFT SHOULDER. HIS FIRST LOST WORK DAY OCCURRED ON 4/6/04.
EE WAS WORKING IN SHOP REPAIRING DRAG BUCKET, FOOT SLIPPED AND HE BANGED IT REALLY HARD INTO THE LARGE BUCKET CORNER. FOOT WAS NOT BROKEN - BUT WAS BRUISED AND SWOLLEN FOR SEVERAL DAYS. EE WAS WEARING STEEL TOE SHOES.
EE WAS DISMOUNTING NUMBER 17 JEEP (2 BLOCKS OUTBY 8A SWITCH) APPROXIMATELY 97 BLOCK 6 NORTH TRACK ENTRY - EE STATED THAT WHILE STEPPING OFF OF JEEP TWISTED RIGHT ANKLE - FRACTURING BONE IN FOOT - BOTTOM UNEVEN - WALKWAY WAS CLEAR OF OBSTRCUTIONS.
Employees were fabricating stacker/ conveyer sections. EE was uncoupling attachment from stacker eye. EE slipped.
The employee was working in an escape raise preparing it for the installation of a liner. He dropped his drill steel and climbed to the bottom of the raise to get it. At that time a piece of rock fell out of the raise from an unknown location and hit him on his hardhat causing his hardhat to cut his head. He received 8 sutures and returned to work that shift, full duty.
Employee opened crusher to inspect, while turning rotor, hand was caught between blowbar and side plate.

EE WAS PULLING AND PUSHING ON TWO WRENCHED AT THE SAME TIME. WHEN THE FITTING LOOSENED SUDDENLY, HE FELT A SHARP PAIN IN HIS RIGHT SHOULDER. HE DID NOT SEEK MEDICAL HELP UNTIL APRIL 9TH AND DID NOT MISS WORK UNTIL TODAY.
EE WAS CUTTING & WELDING ON BUCKET FOR 992G ENDLOADER WHEN FOREIGN OBJECT GOT INTO RIGHT EYE. EE WAS USING WELDING HOOD & SAFETY GLASSES. EYE DOCTOR REMOVED A SMALL PIECE OF METAL AND TOLD EE TO STAY HOME NEXT NIGHT.
Employee was operating a forklift in baghouse when he backed into palletized sand pallets knocking them over. EE proceeded to clean up broken bags and restack pallets. He apparently hurt his back while re-stacking bags. EE finished shift indicating he was not injured.
