Cable insulation on a 12KV electrical feeder line caught fire at 2 spliced areas. The fires consumed approximately 2 feet of insulation at both fire points. The fires were extinguished once it was confirmed that the electrical current was shut off. MSHA Inspectors were on site and were immediately notified of the situation.

There was an accidental entrapment of an individual on the sentinel mine elevator for more than 30 minutes due to an area wide power outage by the failureof a substation. A generator power source was connected to the controls and the individual was removed from the elevator.

THE AREA WAS MINED LATE MARCH 2004 WITH CONTINUOUS MINER AND SUPPORTED WITH 7' POINT ANCHOR ROOF BOLTS. THERE IS A CLAY VEIN RUNNING LENGTHWISE THROUGH THE FALL AREA.

Workers were preparing to move an abandoned fuel tank. The tank had been previously drained and flushed. The workers were preparing lifting using a torch. Stray sparks from the torch ignited pine saw in the area. The fire spread quickly due to windy and dry conditions.

A roof fall occurred in the #4 entry face on the 8 left panel (005 section) at #49 crosscut. the fall was approx 40' long x 18' wide x 7' thick and fell outby the pillar line and entrapped two mobile roof supports. 5' high tech bolts had been installed in the area, supplemented with 12' super bolts.

A roof full in the 13 west panel #65 x cut #4 entry meaning approx. 50' x 20' x 6'-9'. the area was mined in March 2004 and supported with 6' temper rebar roof bolts and 9' mega bolts. the fall was caused by slip.

An excessive rainfall on Friday, March 26, 2004, caused flooding of the quarry at this location. We do not know at this time when normal operation is expected to resume.

002 Section was taking the 21st cut in pillar #5 of Perry County Coal Corp's approved pillar plan when they experienced a power failure from a burned connection in a junction box which disabled the Eimco Dash-1 miner. Miner was not able to back out of cut which allowed roof to fall on front of miner. (Retriever was used to free miner)

An unintentional roof fall occurred on the unit 3 working section due to the strata broke at and below the anchor point. An investigation will be made in all adjoining areas for adverse conditions. The fall occurred at the 4615 foot mark of the #2 entry. The area was originally supported with 5 and 6 foot conventional type roof bolts.

The #1 shield was pulled in on 3 west LW MMU 010, and rock came off t he shield, knocking out timber between the shield and crib built in the walkway, the distance measured 6 1/2 feet between shield and crib. The time to replace the timber was 18 minutes, impeading travel off the face.

The Cedar Creeks service elevator was out of service from aprox. 12:15pm - 5:00pm due to a bad power board. The electrician who conducts the inspection was conducting his inspection at the time of the board failure.

At 67685 158W panel 40 B there was ground failure of 20' x 8' x 10'. We finished mining this panel (remote mining) in January 2004. The area had been inactive since January. We re-entered the top sill of the area this week to install sandline and discovered the failure. Pulled beyond bolts.

The Tyro Creek service elevator was out of service from approx. 5:10pm - 7:30pm due to a problem with the governor rope causing the tail sheave switch to trip.

A roof fall occurred in one of the two intake air courses 3000"" from the active working section. The fall was 1 break long 18'wide and 4'thick. The fall did not impede travel or impair ventilation. The location of the fall was 60'inby SS1330.

A roof fall was found in a worked out area of 5L returns on 3/23/04. The fall damaged a stopping. Fall was in a four way intersection in entry 2 at survey 21 + 00. Area was mined 10-13-00 and supported with 8' full resin bolts. No injury or property damage resulted. no clean up is planned area will be sealed off 4-6-04.

A SUBCONTRACTOR WAS USING A CUTTING TORCH WHEN THE SLAG OFF THE TORCH FELL BELOW THE WORK AREA ONTO SOME MATERIALS. THE MATERIALS BEGAN TO SMOLDER, CREATING A SMALL FIRE. NO INJURIES WERE ASSOCIATED WITH THE INCIDENT.

The mine experienced higher than normal methane levels in the No. 3 Entry of HG-204. Gas was controlled with conventional ventilaiton measures. Further investigation revealed that the mine had inadvertently mined into a horizontal degas borehole.

A ROOF FALL WAS FOUND IN A WORKED OUT AREA 5L RETURNS ON 3/23/04. THE FALL WAS ADJACENT TO THE EXAMINERS TRAVEL ROUTE. FALL WAS IN A FOUR WAY INTERSECTION IN ENTRY 1 AT SURVEY STATION 21+70. The brake thruster assembly on slope hoist in engine room would not operate properly. The brake thruster assembly was replaced and the slope hoist was placed back into service at 5:45 PM on March 23, 2004.

EE was drilling in the #2 face of the west mains MMU 002. The roof had fallen in a dome slope while being mined and measured to be 14' high. Roof mats and screen were installed, while drilling for a roof bolt, the wrench heated the roof pan and sparks from the wrench resulted in a small ignition of methane.

A roof fall was discovered in the no.2 bleeder entry of 1 right off 2 left- 1 right sub off 1 left sub off 2 east, just inby survey station 1008. The fall measured 60'long X 14'wide X 5'thick. There were no injuries or equipment involved. Ventilation & the passage of persons not affected.

A fall of roof occurred in the face of #4 entry extending out by intersection at break #109 on the 002 section. No injuries occurred and no equipment involved. The fall was approx 9' high 30' long and 40' wide in the intersection.

While pulling pillars the Joy 12-12 miner was caught. Rock fell on the miner. The fall was approx. 15ft x 20 ft. wide. 6 to 7ft thick. The miner was caught but not damaged.

Mine Examiner EE was making 002 sections returns and found the rockfall at approx. 10:00am. The fall began in Break 14 #1 entry on 2 West submains and fell down the #1 entry of the #2 North parallel main approx. 1 break. The fall measured approx. 100' x 20' x 5'.

A ROOF FALL APPROX 5' HIGH, 18' WIDE, 40' LONG OCCURRED IN THE #3 INTAKE ROOM OF 10+ LEFT BUTT. A CLAY VEIN WAS EVIDENT & THE AREA WAS BOLTED WITH 4' FULLY GROUTED RESIN BOLTS. FALL WAS NOT CLEANED UP SECTION MOVED TO ANOTHER SET OF ROOMS.

Due to roof conditions and and floor heave the #1 shield tip on the 3 west LW MMU 010 was resting on the H6 drive. Wooden crib was installed to protect the travelway along the stageloader. Rock was removed off the shields to raise the #1 shield. An MSHA investigation found that safe passage may have been impeded off the face.

AN UNINTENTIONAL ROOF FALL OCCURRED IN E-9A SUB-MAINS IN THE NO. 4 ENTRY (BELT) AT SPAD 9672. THE FALL MEASURED 50 FT LONG, 17 FT WIDE AND 8 FT HIGH.

A roof fall occurred on the 2L4W-1L2L at cross cut 9-10 in entry #6. The fall was inby the section tail piece on #3 unit. Cause of the fall was weak laminated shale configured in a stack rock syle.

An experience contractor employee failed to recognize the fact that a belt, made of combustible rubber material, within an elevator housing would catch on fire while they were using a cutting torch to cut through said elevator housing. Burnt belt within elevator.

Equipment fire of D11R dozer. Operator parked equipment at 2:50 am. Mechanic and fueler then proceeded to fix handrail and fuel equipment. No welding or torch was used to fix handrail. Greaser then noticed dozer on fire. all fire extinguishers were spent along with the fire cart.

A ROCK FALL WAS FOUND ON 03/16/04 IN THE C-3 #4 ROOM APPROX. 40' INBY SS871, MEASURING APPROX. 100' W LENGTH, 5-20' WIDE, AND 4' HIGH. A CUTTER WAS OBSERVED IN THE #3 AND #5 ROOM ADJACENT. FALL DOES NOT INTERFERE WITH VENTILATION OR ESCAPEWAYS. ALL ACCESSES ARE DANGERED OFF.

A FALL OF ROOF OCCURRED AT CROSSCUT 53, ENTRY 5 IN THE 1ST NORTH SUBMAIN OFF THE MAIN EAST. FALL MEASURED APPROX. 16' X 16' X GREATER THAN 9' ABOVE THE ANCHORAGE. THE FALL OCCURRED IN AN ALTER NATE ESCAPEWAY.

On the morning of 3/13/04 an unplanned roof fall was discovered in grassy creek #1 mine. The fall occured in #8 & #9 entries spad #'s 5653 & 5669. The size of the fall measured 70'long, 20'wide & approx. 8-9'high. There were no injuries. All approaches to the fall have been supported.
