THE TYRO CREEK SERVICE ELEVATOR WAS OUT OF SERVICE FROM APPROX. 7:13 P.M. UNTIL 8:00 P.M. DUE TO A GROUND FAULT TO THE BUILDING.

A ROOF FALL OCCURED IN ENTRY #4 MAIN SOUTH @ SURVEY STATION 31 + 10. THE FALL MEASURED 120' LONG X 19' WIDE X 6' THICK. THE AREA HAD BEEN BOLTED WITH 5' FULLY GROUTED BOLTS. THIS AREA WILL BE CLEANED UP AND REBOLTED.

During the weekend of 5/29-5/30 the hoist basement took on water during hard rainfalls during short periods of time. The water submerged brake pumps and the drum gear pump. The basement was pumped dry, the pumps were disassembled, cleaned, baked, replaced and tested, and the hoist was tested and placed back into service at approximately 1:45 pm on 6/1.

THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROXIMATELY 119 MINUTES. THE POWER WAS OFF. IT CAUSED THE ELEVATOR CALL SWITCH TO GROUND AND PUT IT IN STOP.

Diesel backup pump to control incoming water, it kept up until about 7:30 pm at which time the mine started taking on water at return entrance, no one was in the mine and the power was restored at 8:12 pm.

A SHORT CIRCUIT OCCURRED IN THE HEADFRAME WIRING TO LIMIT SWITCHES. THE MINE WAS EVACUATED AT 1:40 PM & COMPLETED AT 1:55 PM. THE WIRING WAS REPAIRED & THE HOIST WAS TESTED. MINERS RETURNED UG AT 2:20 PM.

A ROOF FALL WAS DISCOVERED IN THE #2 ROOM OF THE 1ST SOUTH BUTT SECTION. FALL WAS FROM #4 X-CUT TO #6 X-CUT AND THROUGH X-CUT TO #3 ROOM. FALL AREA WAS 135' LONG BY 18-20' WIDE AND AT LEAST 6' HIGH. AREA WAS BOLTED WITH 4' FULLY-GROUTED RODS. THIS WAS A KNOWN BAD ROOF AREA. THE ROCK FALL HAD LAMINATIONS AND STREAKS OF SANDSTONE.

Fall app 2500' out by 001-0 unit app 18' long 18' wide and 8' high. No equipment involved, no blockage of ventilation or escapeway, no persons involved, will not be cleaned up. Dangered and timbered off and cribs set. Marked on map.

HAD SMALL ROOF FALL 1 BREAK ABOVE SPAD #4076 IN THE INTAKE AIR COURSE #4 ENTRY IN A TRANSITION AREA OF SLATE TURNING TO SANDSTONE (FAULTY AREA). THE FALL WAS APPROX 5'HIGH, 18'WIDE AND 20'LONG. BOLTED WITH 4'5/8 RESIN BOLTS. IT'S NOT GOING TO BE CLEANED UP. WE HAVE A SAFER TRAVEL WAY AROUND IT AT TIME OF INVESTIGATION AND IT IS NOT RESTRICTING THE AREA.

This roof fall occurred in an area that had previously been abandoned. It was discovered while doing the weekly examination of the intake and return airways. The area has been cribbed off and has no adverse effect on the ventilation. the fall was approx 20 ft long by 15 ft wide by 8 ft high.

ROOF FALL ON #002 SECTION IN #2 ENTRY AND BACK TOWARD #3 ENTRY. FALL WAS APPROX. 80' LONG, 20' WIDE AND 5' THICK. TOP WAS BOLTED WITH 5' POINT ANCHOR BOLTS. FALL WAS IN FACE 2 BRKS INBY SPAD 29073 AND WILL NOT BE CLEANED UP.

AN UNINTENTIONAL ROOF FALL OCCURRED AT E-MAINS ALONG #10 BELT, BETWEEN #5 & #6 ENTRIES, AND SPADS 9402 & 9403. THE FALL MEASURED 8' HIGH X 30' LONG X 18' WIDE.

Roof fall in the #7 entry at 3 way intersection 4+68. The piece of roof that fell was approximately 30' long. This area has dripped water from the time of initial mining to the time of the fall.

AN UNINTENTIONAL ROOF FALL OCCURRED AT E-MAINS, ALONG #10 BELT, BETWEEN #5 & #6 ENTRIES (BETWEEN SPADS 9402 & 9403). THE FALL MEASURED 8'HIGH X 30""LONG X 18' WIDE.

An excessive rainfall overnight, May 24, 2004, caused flooding of the quarry at this location. We do not know at this time when normal operation is expected to resume.

Failure of ARCNET communication card on the #2 hoist drive of the production hoist. Mine was evacuated. Card was replaced and hoist was tested and back in service by 7:50am. No injuries.

A ROOF FALL WAS FOUND 60 FEET INBY SPAD #2825 DURING THE FOREMAN'S INSPECTION OF TRAVEL WAY. THE FALL WHICH IS LOCATED OUTBY THE WORKING SECTION MEASURED 6' X 18' X 35' AND WAS IN AN AREA THAT HAD PREVIOUSLY BEEN DANGERED OFF.

A roof fall was found 2 breaks inby spad 24605 between the track and intake entry. The fall measured 80' L X 20' W X 7' H. and occurred due to a rider seam above the immediate roof. Roof anchorage in the area was 4 foot resin bolts. No persons were injured as a result however, ventilation was affected. The fall was dangered off and ventilation was re-routed around the fall.

A ROOF FALL OCCURRED ON #2 UNIT, 54 CROSSCUT, #3 ENTRY INTERSECTION. THE FALL WAS IN A TRANSITION AREA WHERE ROOF ROCK WAS CHANGING FROM SANDSTONE TO SHALE. THE FALL MEASURED APPROX. 30'LONG X 17- 18'WIDE X 6-7'HIGH. THE AREA WAS SUPPORTED USING 5' FULLY GROUTED ROOF BOLTS & 12'CABLE BOLTS.

AREA SUPPORTED BY 8'FT LONG POINT ANCHOR BOLTS. ESPANDER STEEL & 2 F'RESIN. HEADER BLOCKS & WIRE SCREEN WERE USED IN CONJUNCTION WITH THE ROOF BOLTS. THE AREA BOLTED ACCORDING TO PLAN

A ROOF FALL OCCURRED IN THE #3 SECTION RETURN SOMETIME DURING THE WEEK OF 5/24/04 TIL 6/3/04. THE FALL MEASURED 40' LONG X 18' WIDE X 7' THICK. THE AREA WILL NOT BE REHABILITATED. THE FALL WAS 1 BREAK OVER FROM SPAD #13203.

AN UNINTENTIONAL ROOF FALL OCCURED AT THE MILENNIUM PORTAL MAIN WEST ENTRY #2595W, CROSSCUT 19. THE FALL WS APPROX 35'L X 191/2'W X 10-13"" THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF PEOPLE. FALL WILL BE CLEANED UP.

A ROOF FALL WAS DISCOVERED AT 3:00 AM BY THE FIREBOSS. THE ROOF FALL WAS LOCATED IN THE NO.1 ENTRY AT SPAD NO. 9686. THE FALL MEASURED 70'X 19.5'X 8'THICK.

Roof fall on the 8C belt entry brk 32 out by spad # 28825, fall was approx 20' wide by 30' long by 6-8' thick. Top is laminated slate bolted with 4' glue bolts. Fall will be cleaned up.

A PROPANE TANK WAS BEING BLED OFF WITH THE GAS BEING BURNED. A FIRE WAS REPORTED BECAUSE OF POOR COMMUNICATION. THE FLAME GAS WAS NOT TOXIC. IT WAS 200'FROM THE FRESH AIR INTAKE. THE MINE WAS EVACUATED AS A PRECAUTIONARY MEASURE.

A roof fall measuring approx 10' w by 20' l by 3 1/2' thick was discovered in the conveyer entry on the morning of 5-21-04. The area was cleaned up and cribbed and cross collared. the fall was located between #2 and #3 crosscut in by the portal.

EE was in process of disconnecting charger plugs from scoop batteries when an apparent hydrogen ignition occurred at the batteries.

AN UNINTENTIONAL ROOF FALL OCCURRED AT 27 BLOCK IN INTAKE, #9 ENTRY, AT SPAD 255, ACROSS FROM #1 BELT IN AN INTERSECTION. THE FALL MEASURED 6' HIGH X 18' WIDE X 18' LONG.

A ROOF FALL WAS FOUND AT THE BEGINNING OF THE SHIFT. THE FALL MEASURING 5' X 18' X 25' WAS LOCATED APPROX. 250 FEET INBY THE CONVEYOR BELT PORTAL OF THE CONVEYOR BELT USED TO TRANSPORT COAL TO THE LOADING FACILITY. NO INJURIES.

A roof fall occurred out by #2 unit. The fall was in a return air course. The fall was in entry #8 at 17 crosscut. The fall is approx 35' long, 18' wide, 10' high. It was bolted with 6' and 10' point anchor roof bolts. The fall will not be cleaned up.

The Main Portal elevator became inoperable. Investigation revealed a faulty door switch. The door switch was repaired and the doors were adjusted. The elevator was placed back into service at 5:40 PM

THE HALLAM ELEVATOR COULD NOT BE CALLED OR DISPATCHED FROM ONE LANDING TO THE OTHER DUE TO A RE-SET PROBLEM WITH THE BODIE BRAKE CIRCUIT.

THE ELEVATOR PREMATURELY STOPPED IN THE SHAFT DUE TO CONTROL CABLE BECOMING DETACHED FROM THE MID SHAFT JUNCTION BOX. THE CONTROL CABLE WAS REPOSITIONED & RECONNECTED. OPERATIONAL CHECKS INSURED THAT THE ELEVATOR WAS FUNCTIONING AS REQUIRED.

ON 5/20/2004 THE ELEVATOR AT THE JUSTICE MINE WAS DOWN FOR APPROX. 45 MINUTES DUE TO THE TERMINAL END WAS BURNED OFF AND GROUNDED. THIS WAS REPAIRED, BUT THE SAME PROBLEM OCCURRED AGAIN AT APPROX. 4 A.M.

THE #1 ELEVATOR BECAME DISABLED AT APPROX 4:15 A.M. ON 5/19/04. THE CONTACTORS AND CONTROL RELAYS WOULD CHATTER WHEN ENGAGED AND THE CAR WOULD NOT RUN IN NORMAL SPEED. A BUFFER SWITCH WAS FOUND TO BE OUT OF ADJUSTMENT AND HAD WATER IN IT. THE SWITCH WAS DRIED, A NEW LID INSTALLED AND SEALED ON SWITCH.

An unintentional roof fall was discovered at the mouth of a 4 west off north mains at spad #8683. fall dimensions were estimated at 60 ft in length by 2o ft wide by 8 ft thick.

THE MIRACLE RUN ELEVATOR WAS SENT TO THE BOTTOM, DOORS OPENED BUT WOULD NOT SHUT. THE SHINDLER REPAIRMAN FOUND A FAULTY DOOR LOCK MECHANISM & ONE BAD LEAD ON TRAILING CABLE GOING TO ELEVATOR CONTROL PANEL.

A roof fall occurred on the 3MN belt at cross-cut 12 in entry #4 of the 3MN. The cause of the fall was weak laminated sandstone. The fall covered the 3MN belt and structure. The fall was cleaned up and the area rehabilitated. The fall was found on the preshift examination.

A roof fall occurred in the south west mains at survey station 8 and 80 #3 entry. The fall measured 30' long by 18' wide by 6' thick. This area had been bolted with 5' fully grouted bolts. All approaches have been timbered off.

WE HAD A ROCKFALL IN ONE OF THE 2 NEUTRALS ABOUT TWO THIRDS OF THE WAY UP BESIDES #8 BELT AT SPAD #4566 MEASURING APPROX. 20' WIDE, 40' LONG AND FROM ABOUT 4' TO 12' HIGH. IT BOLTED GOOD WHEN WE MINED THROUGH IT BUT APPEARS TO BE AN UNDETECTED FAULT AREA. REPORTED IT TO MINE INSPECTOR.

A roof fall occurred in the 1st north, #8 entry, #19 crosscut. The fall was 40' long, 20' wide, and 8' high. The fall did not affect travel, ventilation, damage equipment, or cause any injury. The area was bolted with 6 and 8 point anchor bolts.

The #8 entry out by spad #1091, a roof fall occurred at an intersection measuring 18' long and 18' wide. The fall was approx 5' high, 5 foot resin bolts were being used. the roof appeared to be normal at the time it was mined. 7 foot test holes were installed in pattern.

AN UNINTENTIONAL ROOF FALL OCCURRED IN 8 MINER MAINS IN #6 ENTRY, IN AN INTERSECTION NEAR SPAD 10547. THE FALL MEASURED 25 FT. WIDE X 25 FT. LONG AND 6 FT. HIGH.

A roof fall occurred on the 3B belt at cross-cut 4 in entry #4 of the 2L4W unit. The cause of the fall was weak laminated shale. The fall covered the 3B belt and structure. The fall was cleaned up and the area rehabilitated.

A roof fall occurred at Diamond Energy possibly the day of 5/17/2004 or before. The time is unknown. This fall occurred in the area of spad 2385 in an entry between headings 3 and 4. and measured 16' x 18' x 6'. and was originally secured with 8' cable bolts. This area is being rehabilitated as part of it is in the track entry area.

BASED ON THE INFORMATION IDENTIFIED BY THOSE PRESENT, IT APPEARED THE IGNITION WAS CAUSED WHEN SPARKS FROM THE CUTTING MACHINE (SHEARER) TAILGATE DRUM IGNITED AN UNKNOWN METHANE BLEEDER WHILE CUTTING COAL/ROCK. THE FLAME LASTED LESS THAT 3 SECONDS.

A roof fall occurred measuring 18' wide and 7' high and 8' wide in by spad #118 3,000' out by working place. No equipment was involved, no ventilation was disturbed no injury occurred and the fall will not be cleaned up.

A roof fall occurred in #8 entry of the south west mains at survey station 13 and 60. The fall measured 20' long by 18' wide by 6' thick. This area had been bolted with 5' fully grouted bolts.

AT APPROX. 2:10 A.M. ON MAY 17, 2004, A GROUND FALL WAS DISCOVERED IN THE 13 NORTH RETURN AIRWAY AT 5 W INTERSECTION. FALL MEASUREMENTS WERE APPROX. 45' WIDE X 70' LONG X 0-8 1/2 FEET THICK. THE FALL AREA WAS TRAVELABLE ALONG THE WEST RIB. MSHA FIELD OFFICE NOTIFIED.

INCIDENT NOT AN ACCIDENT. WAS NOTIFIED SUNDAY BY PUTNAMVILLE FIRE DEPT. THAT OUR MACHINE WAS ON FIRE. SMOKE NOTICED BY FIRE DEPT. AND WAS EXTINGUISHED. CAUSE UNKNOWN AT THIS TIME. NO PERSONNEL FROM RED TOP RENTALS OR HANSON AGGREGATES WERE PRESENT AND NO INJURIES OCCURRED.

Rock fall in one of the neutrals about 1/3 way up #8 belt area(not in belt entry) at spad #4423. Timbered it off & made roadway around rock fall into the other neutral rock fall. Fell sometime between 2pm Sat 5-15-04 & 8pm Sun 5-16-04. Rock fall was approx 20' wide, 40"" long & from 4' to 10' high, it bolted good at the time it was mined w/4' resin bolts.

ROOF FALL IN THE 5 EAST DEVELOPING SECTION, #3 ENTRY FROM 25 1/2 TO INBY CORNER OF 26 CROSSCUTS. FALL IS APPROX. 80 FT LONG, 17 FT WIDE, 8 TO 10 FT HIGH. AREA WAS MINED LATE APRIL '04 WITH CONTINUOUS MINER & SUPPORTED WITH 7' POINT ANCHOR BOLTS SUPPLEMENTED WITH 12 & 16 FT. CABLE BOLTS. THERE WAS SOME ROOF TO GROUND SUPPORT INSTALLED(CRIBS). AREA HAS CLAY VEIN RUNNING WITH ENTRY.

At 4:25 p.m. the main portal elevator doors would not function properly. Troubleshooting revealed that the north safe edge switch and linkage were out of adjustment. The door linkage and switch were corrected and the elevator was placed back into service at 5:40 p.m.

A ROOF FALL OCCURRED IN THE #5 INTAKE ENTRY ON THE #2 SECTION. THE FALL WAS ONE X-CUT OUTBY THE FEEDER. THE FALL WAS APPROX. 6' THICK X 20' LONG X 20' WIDE. THE AREA HAS BEEN DANGERED OFF & WILL NOT BE REHABILITATED.

A ROOF FALL WAS DISCOVERED IN THE BELT ENTRY FOR 001. THE FALL IS 2 CROSSCUTS INBY SPAD 39. THE FALL IS APPROXIMATELY 18' WIDE X 70' LONG X 10' HIGH. THE AREA IS TO BE CLEANED UP AND RESUPPORTED WITH CRIBS, CROSSCOLLERS AND ROOF BOLTS AS CLEANUP IS ADVANCED.

A roof fall occurred on the South Mains A belt No. 4 xcut. The fall was approx 4' high by 18' wide by 30' long. Fall did not obstruct ventilation or escapeway. The fall stopped the belt. Area was re-supported and cleaned up. Cause of the fall was beam failure from the roof bolts creating the beam.

The injured person, an employee of Service Transport, was delivering diesel to the quarry fuel tank. The driver was the only person at the tank when a hose separated from the truck and he was burned by the hot diesel fuel. He was taken to the hospital with burns - described as non life threatening. Driver later died from what is believed to be complications from the injury.

Employee was installing new corner bits bolts in dozer blade. He had all old bolts removed and put a bar in bolt hole to hold bit in place. The bar slipped and the corner bit fell striking his left foot at the base of his toes. Three toes were surgically removed, the big toe and the next two.

EMPLOYEE WAS DIAGNOSED WITH HERNIATED DISC AND WAS GIVEN AN INJECTION ON 6/20/04. HE HAD BACK SURGERY ON 8/9/04 MAKING THIS A LOST TIME INCIDENT.

LOADER OPERATOR NEVER SAW EE & KEPT GOING, EE ROLLED BETWEEN FRONT LOADER TIRES, FRONT LINE EDGE RAN OVER EE'S LT. ARM FRACTURING BONE.

Employee stated he was servicing and greasing the #410 RBM using a hand held battery grease gun when a small piece of draw rock fell striking his right thumb and forefinger. The sharp edges of the rock caused lacerations to the thumb and forefinger and injured a tendon to the thumb.

Flash burns (welding)

Employee was helping repair/weld on new wear liner in WA500 loader bucket. Employee welded approximately 30 minutes and was a helper for the remaining time of repair. He received arc flash burns to both of his eyes.

Employee was cutting swivel pin out of miner. A piece of hot slag came back and burnt the oxygen hose in two right behind the torch causing 2nd degree burns to right hand and fingers.

The ee was transfering rock dust from a mobile tank to a pressurized holding tank. The compressor for the duster was not de-energized and the pressure not bled off, when the cap was unlatched it blew off and struck the ee in the face.

EE WAS OPERATING A HAUL TRUCK. THE RIGHT FRONT STRUT BROKE ON THE TRUCK. THE RIGHT FRONT OF THE TRUCK HIT THE FRONT TIRE. EE SAID HE HURT HIS BACK. THE BROKEN STRUT LET THE TRUCK SIT DOWN ON THE TIRE.

Ee reported: He was attempting to remove a damaged bit holder from 14cm15 cont. miner on No. 5 unit. Due to angle of bit holder's attachment to machine's drum, he needed to use a bit holder remover to loosen holder from drum cone. When he struck holder remover using a 20 lb sledge, a sliver of steel broke off penetrating his right thigh.

Ee standing in operator's compartment on operator's seat, lifted left leg to step on top of machine. The seat was not attached and slipped from under his weight. He fell backwards hitting his right side on edge of operator compartment.

INJURED WAS WALKING DOWN INTO THE TUNNEL TO GREASE MAG PUMP WHEN HIS RIGHT FOOT SLIPPED, CAUSING HIM TO TURN HIS RIGHT KNEE. HE WAS SEEN BY A PHYSICIAN ON 5/10/04. AN MRI WAS PERFORMED ON 5/12/04 AND CAME BACK POSITIVE. SURGERY WILL BE PERFORMED DURING HIS REGULAR VACATION SHUTDOWN TIME.

The employee had an MRI on 5/13/04, the results were read on 5/17/04 and she was removed from duty on 5/18/04. The accident did not become reportable until she missed her first shift on 5/18/04.
