SOUTH SPEAR POINT BROKE LOOSE FROM ANCHORAGE POINT CAUSING SKIP TO HANG UP IN LOADING POCKET. #3 HOIST WAS UNAVAILABLE FOR APPROX. 8 HOURS.

The slope car was inoperable when the braking system malfunctioned. The car services the mine slope and is part of the mine escapeway system from the West Mains 020 section. All affected personnel were informed to use the belt escapeway until repairs were made. No injuries resulted from this hoisting incident. AMENDMENT: NOT an accident but rather, planned maintenance.

AT APPROX 1:35 PM THE MIRCLE RUN ELEVATOR LEFT BOTTOM WITH ONE PASSENGER. THE ELEVATOR STOPPED AT ABOUT MID SHAFT. THE ELEVATOR WAS OPERATED TO THE SURFACE AND THE EE EXITED. THE S-O-S SWITCH WAS RE-SET AND ELEVATOR RETURNED TO OPERATION AT 4:05 PM.

Guide rails were being changed in shaft. While postioning the work platform it caught on guide causing damage to platform. This was during a maint. outage and no prod. was taking place in the mine. Only 13-15 employees were in mine. Repairs made & test run at 4:00am, 5-1-05.

A fall occured on the 8 west section in the intersection of the #4 heading, #9 crosscut (spad 13+37). This fall extended over into the #4 to #5 crosscut. No one was injured.

A roof fall occurred on the 3C panel at cross cut 6, entry #2, outby location spad set at 4+83.14. The cause of the fall was weak laminated shale. The fall is located in the return air course for MMU-003 and is located adjacent to the return stopping line.

Elevator will be down for scheduled maintenance on 5/1/2005 through 5/5/2005. Rails which the elevator car travels will be replaced. Man hoist as well as the emergency hoist will be manned and ready 24 hours a day until elevator is returned to service. A call to the Lexington Kentucky MSHA office was made on Friday Arpil 29, 2005 and spoke with Randy Dotson informing of the outage.

A roof fall occurred in the 9 North Panel #3 entry in the pillar line between cross-cut #37 and cross-cut #38 and extending through the intersection of cross-cut #38. The fall measured approximately 55ft. x 19ft. x 5ft. The area was supported by 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause of the fall. The area was mined in April 2005.

A large roof fall occured in DD @ 11 intersection, luckily the scaler or any other personnel were not in the area at the time. The operator had intentions of scaling a large rock when the machine broke down on the 27th.

Employee was pushing coal to raw coal feeder with a dozer when he pushed into a cavity causing coal to slide in around the dozer keeping him from backing up or open the doors until the coal was removed. Employee was not injured, equipment was not hurt.

A roof fall occurred at survey station 42+00. The fal measured 15' long x 18' wide x 9' thick. This area had been supported with 6' fully grouted and 9' double lock bolts. All approaches have been timbered off.

OPERATOR CLEANING FACE ON 1600'LEVEL OF MINE. A NON-FATAL FIRE STARTED ON LOADER THAT COULD NOT BE EXTINGUISHED, THE MINE WAS EVACUATED. FIRE WAS LATER DETERMINED TO BE CAUSED BY ELECTRICAL COMPONENTS.

A roof fall occurred in the 9 North Panel #9 entry in mid pillar between cross-cut #30 and cross-cut #31 extending inby through the pillarline between cross-cut #32 and cross-cut #33. The fall measured approximately 90ft. x 19ft. x 5.5ft. The area was supported by 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

At approximatly 12:30 pm a ground fall was discovered at the Deep Post mine in the Deep Star Decline. It was located 75 feet above the second set of steel sets going up the ramp. The demensions were 9 high 12 wide 14 feet long. This was reported to MSHA. Rehab of the area has begun per MSHA approval.

An unintentional roof fall occurred on the pillar line of 002-section which temporatily caught 2 mrs's both were recovered fall area abandoned.

Unintentional roof fall occurred in the No. 3 Hdg that pulled 3 rows of permanent roof supports. Fall was 45' long x 20' wide and approx 15' high.

Hoist down from 11:30 am to 4:30 pm. Coil in main contactor and surge suppriss burnt. repaired approximately 5:00 pm. Hoist down second time from 6:15 to 4:30 am in coil in main rectifier. No injuries reported as a result of this hoist being down.

The fire occurred at the remote scrap metal salvage yard and involved two reels of oil impregnated wire rope that were being cut up for scrap by FMK (ID H142). The incident occurred after the shift ended. The fire was extinguished by the local FD ~45 minutes after notification. The salvage yard is remote from the operating site, structures and operator work stations.

A roof fall occured in the #9 North Panel, #7 Entry, starting half way through the intersection of crosscut #16, proceeding half way through the intersection of crosscut #18. The fall measured approximately 110'x19'x5'. The areas was supported with 5' fully grouted roof bolts and 9' double lock mega bolts. An undetected slip was determined to be the cause of the fall.

DEVELOPED 2/05, 4WAY INTERSECTION OF #6 HEADING. FELL IN XCUT 5-6 & 6-7 ALSO FELL. #7 & #5 INTERSECTIONS HAD CEMENT PIERS BUILT IN THEM & NEVER FELL. FALL WAS APPROX 17'W X 4'H X 70'L.

On Friday, April 22, beginning at approx. 6 pm, three power outages occured. Upon restarting of the #3 kiln coal firing system, an explosion occured within the mill or ductwork. The safety rupture disc burst as designed. No personal injuires or property damages were sustained.

A roof fall occurred on the 3C panel at cross cut #9 in entry #2. The cause of the fall was weak laminated shale often referred to as stack rock. The fall was located in a return air course located outby the loading point for the section.

THE MIRACLE RUN ELEVATOR STOPPED IN MID SHAFT ON THE WAY TO THE SURFACE. ELEVATOR REPAIRMAN FOUND AN INOPERABLE CONTACTOR IN THE MAIN CONTROL PANEL. ONE PERSON WAS INSIDE OF THE ELEVATOR FOR 20 MINUTES.

An unintentional roof fall was found at approximatley 6:48 pm. The fall measured 5'hx30'wx25'l. The fall was dangered off, Roof Fall Cleaning Procedures were reviewed and posted at the fall and proceded with the clean-up. The fall was in the #6 Entry, inby spad station #3756, Mains Section.

An unintentional roof fall occurred at the 4north headgate entry #1, 2237s, corsscut 2. the fall was approx. 35' long, approx. 18' wide and approx. 16' thick. the fall did not affect ventilation or passage ofmen. the fall will not be cleaned up.

THE TANKER FROM WHICH AMMONIUM HYDROXIDE WAS TO BE EXTRACTED DID NOT LOOK RIGHT TO AN EE AND EE PLACED HIS HAND ON EXTRACTION VALVE ASSEMBLY IN THE AREA OF THE WING NUT ON THE CLAMP THAT HOLDS BONNET PORTION OF VALVE IN PLACE. A ROOF FALL WAS DISCOVERED IN THE IST NORTH SUB-MAINS, #1 ENTRY, CROSS CUT #16 INTERSECTION THAT WAS APPROXIMATELY 4 1/2' TO 5' HIGH.

Hoistman went to #2 shaft to send people UG. He noticed there was no power to the hoist. Hoistman failed to notify his supervisor immediately, so there was a delay in getting help to repair hoist. Down 5:30 a.m. to 7:00 a.m.

A fall of roof occured int he #43 longwall panel teardown face. Fall was 14 ft wide x 16 ft highx 132 ft lontg. Area was supported with 16 ft cable bolts, 6 ft combination bolts dywag slings, t-5 channel and 30"" pumpable cribs. No equipment involved and no injuries. Cable bolts were strill in rod as support.

Temporary hoist outage. The service hoist was pulling electricity while stopped at the shaft bottom. Problem discovered at 8:00 am. All underground employees (5) evacuated the mine through the production hoist. Repair work and troubleshooting was complete at 10:15 pm, and employees entered the mine at 10:30 pm. MSHA field office at Newark was notified at 10:25 pm.

A reportable roof fall occurred in 19 Headgate, entry 2, crosscut 19. The fall was approx. 18' long, 18' wide & 7-8' high.

Miracle Run elevator stopped in mid shaft on the way to the surface. The Shindler elevator repairman found a damaged steel cable that closes and opens the automatic doors and a bent sheve wheel that rolls up the excess.

A roof fall was found in the #3 main entry one crosscut inby spad #75. The fall was 20' by 20' by approximately 9 feet in thickness. The area was dangered off, and additional support was set in the area. The fall did not disrupt ventilation or the mining cycle.

Roof fall in K west panel 90 West/206 South. Dimensions of Roof fall: @ 40 ft long by 20 wide and @ 6 feet thick. This area and depth of rock is above the 4 foot anchorage point. This was an extention of a previous roof fall. Unknown time of fall. It was found yesterday on 4-14-05 and notified Lexington MSHA at @ 5pm 4-14-05.

A bearing failed on the #4 belt tail piece, causing dense smoke and high CO preventing access to the immediate area. After the belt was stopped, the bearing heat burned the belt into and started the inpact bed burning. The area was accessed under apparatus and the fire on the belt ends and inpact bed was extingushed within 10 minutes.

On old 1 rt panel (9 belt) #4 by inter fell in, 1 block in by spad #2513. Area developed 12-04, bolted with 6' resin bolts and T-3 channel. it was a 4 way inter, cover approx 300-350'. Fall was approx 6' high 16' l 16' w. Fall will not be cleaned up. Posted and dangered off.

Fall found during weekly check - Fall app. 30 x 18 x 8 - Blocked primary escapeway - Re-routed the primary escapeway - No injuries, no equip. involved, no blockage of vent. Will not be cleaned up - plotted on mine map. App. 5000' from face area.

Fall of ground above the anchorage zone, eastern body bench 79-80, 20' wide by 120' long and 12' to 15' in height. No injury occurred. 980 loader was slabbed hitting bucket.

THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES WITH NO ENTRAPMENT OR DAMAGE TO EQUIPMENT BECAUSE OF A BLOWN 20 AMP FUSE IN THE FIELD CIRCUIT.

A reportable roof fall occurred in 19 Tailgate, entry 2, crosscut 16. The fall was approximately 23 feet long, 32 feet wide and 6 feet high. The roof fell on top of the conveyor belt installed in the entry.

Main Production hoist kicked out and would not restart. Electrical system on hoist was checked and discovered that 1100 amp, 1000 volt fuse had blown. A replacement fuse was installed after delivered to mine site at 10:00 p.m. Mine was evacuated because hoist is secondary escapeway.

A roof fall occurred along the 3B roadway at cross cut 11 between the belt and roadway. The cause of the fall was weak laminated shale often referred to as stack rock. The fall partially covered stored aluminum water line.

AN UNINTENTIONAL FIRE WAS DISCOVERED ON THE STOCKTON COAL STOCKPILE AT 10:00 AM. THIS FIRE MEASURED 6' X 8' AND WAS BURNING AT OR NEAR THE SURFACE OF THE PILE.

Unspooling a 4/0 cable into intake side of elevator shaft the contractors equip failed allowing 1400 ft of cable to unspool & fall into 1/10 the bottom shaft. He assist in cable installating were aboard elevator. No damage Elevator was taken out of service by people on board & unitl inspection was complete. the elevator was out of service approx 90 min.

A fall of roof occurred in the 4th SE submains on #6 unit at x-cut 24 in the #4 entry. The fall was 50' x 20' x 5'. The fall did not affect ventilation, equipment or personnel.

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE 6 NORTH HEADGATE SETUP BETWEEN ENTRY #2 AND ENTRY #3 AT CROSSCUT 40, 2315N. THE FALL WAS APPROX. 25' LONG X 18' WIDE X 6-8' THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF MEN. THE FALL WILL NOT BE CLEANED UP. FALL #22L.

A non-injury roof fall was found by the fireboss on his weekly examination. The fall was in the west mains #1 entry at x-cut 30. The fall measured 25' by 25' and 6 to 6 1/2' high.

An un-intentional roof fall occurred @app. 6:15 A.M. on the section in the last open crosscut intersection of #3 entry on the 001-0 section. No men or equipment were involved. The fall will not be cleaned up. 20' x 20' x 6' with 4 foot resin bolts. Called in to MSHA on 4-07-05

An unintentional roof fall was discovered at 7:10 AM on the mains section. The fall was in the #1 entry, 1 break inby spad #3765. The fall measured 5'H x 20'W x 18'L. Fall was investigated by MSHA inspector at 7:10 AM 4-6-05, and by State inspector at 8:40 AM 4-6-05. The fall occurred in a 3-way intersection.

As he slipped, he grabbed for the handrail with his left hand but could not stop himself from falling off the ladder, 5 feet to the ground. He landed on his rear end which resulted in a fractured vertebra.

Employee was in a manbasket attached to a forklift. The basket was 5 feet above the ground. While lowering the basket, the forks stuck momentarily, the chain continued to move, then the forks fell 3-1/2 feet stopping the basket 1-1/2 feet above the ground. The sudden stop casued the employee to strike his throat on the manbasket handrail.

When one EE struck an arc to weld there was a flash from the escaped gas that burned both employees. This EE had burns on his face and ear.

As the Drilling Rig Assistant, grabbed the Core Barrel with his left hand and placed his right hand under the core barrel, the Core Barrel suddenly dropped, pinching the Assistant's right hand between the Kelley Turntable and the bottom of the Core Barrel.

Operating crane (50 ton grove) rigged to 5700 lbs truss, moving 100' downhill in reverse, load in uphill position. Started to descend air pressure 120 lbs, crane in low gear reverse, traveled 30' to 40' down incline boom full retracted. Lost pressure due to mechanical failure. Brake max ruptured.

Employee was in the process of loading a face round with 3 other miners. While he was bent over loading one of the knee holes, a rock weighing appox. 50 lb fell out of the face striking his hard hat and left shoulder. The rock fell from a height of about 10 feet.

Operator had parked truck at truck shop. She was walking down boarding ladder & stepped off last step before the platform above breakaway ladder. Her left boot heel hung up on edge of step, causing her to fall forward, twisting her knee & ankle. Diagnosed with soft tissue strain. Minimum days off work: 4 weeks. Incident has concluded 5/2/05.

Employee was riding in the passenger seat of a diesel personnel carrier. The driver was passing a diesel duster in the supply road. the personnel carrier hit the duster causing the employee to fall forward hitting his face. His knee swelled up. The x-rays were negative but because of the bruise he missed work for seven days.

EE informed us that he tried to open the door of the blending silo pressurizing fan room to inspect the equipment inside. The suction on the door was so great that while attempting to pull the door open he felt his shoulder pop and burn.
