At 9:50pm, #3 hoist tripped on east motor #2 overtemp. After looking at hoist, it was determined that a bearing needed to be changed. The bearing was changed and hoist was back in service at 2:00 am. There were still two operable hoists at all times.

An uncharted gas well was intersected on the longwall face at 152 shield. Mining was haulted and all power removed. 1.3-1.7 methane was detected.
A roof fall was discovered in the 2 West 4 Mains, at Crosscut 42, between #4 & #5 entries. The fall measures approx 25' long, 14' wide and 5' thick. The area is supported with 5' fully grouted resin bolts and torque tension rebar bolts.
Area of roof at intersection of 1N and 6W fell, impeding travel along the primary escapeway. Fall of roof occurred at start of 3rd shif and no miners were in the mine at the time. This area of the mine was developed prior to 1953 and was never bolted. Area has been scaled and ground support added at edge of the fall zone.
A roof fall occurred in No #5 last open crosscut. Extanded into No. 5 to 4 crosscut. The fall was 8 ft high, 20 ft wide, 60 ft in length. The continuous miner was caught under the fall.
A roof fall occurred at #14 cross-cut, roadway entry of the South Mains. The intersection fell 42 inches high, 20 feet wide and 20 feet long.
Fuel line of left side of dozer punctured and sprayed fuel on turbo, causing the fire.
A roof fall occurred in No #3 intersection. Four to eight ft high, 20 ft wide, 30 ft in length. It was cribbed off.
A foof fall was found at 10PM at x-cut 253xc entry 4, main south. The fall was in a 4-way intersection that had been mined in September 1997 & was supported with 4 & 8 foot full resin bolts. Additional props had been added recently to reduce entry width. No injuries resulted, some cable and waterline were lost. There are no immediate plans to clean this fall up.
A grassland fire starting offsite entered the Palafox and trevion areas of the mine. No personnel, equipment, fuel, or explosives were on site during the fire. MSHA was contacted at 5:52 pm MDT on 3/29/09, call ID No. 1-62054095. Inspect of MSHA returned the call at 6:23 pm MDT
Not an accident, injury or illness. It was an event that met the definition(11)in that hoisting equipment operation(""use of equipment"")was interfered with for >30 minutes. Related to equip. for hoisting ore, not lifting persons out of the mine. The cooling fan motor had failed. For lifting persons, the cooling fan could have been bypassed. 2 hoists still in operation
#3 hoist tripped on vibration trip-west motor. Electrician had to come out of the mine to reset the hoist. The hoist was reset at 2:10 am. There were still two operable hoists during this incident.
We had 16 inches of rain in a 35 hour period which caused a dam to overflow and breach. There are no dwellings or roads in the downstream flow. All water from the dam impoundment flowed into the Tombigbee river swamp and posed no danger to anyone. We reported the breach to an engineering company who advises us on impoundments.
Roof fall was on 6th Southest beltline 95 ft inby station # 7856. Fall was 30 ft long, 30 ft wide & 4 ft high. Area was bolted with 42"" bolts.
An unplanned roof fall occurred in the # 130 mine on # 8 belt near spad 6653. The fall measured 30' long x 25' wide x 5' high. No injuries were reported. The area was developed on 3/31/08.
At 12:15 pm on 3/27/09 Oak Hill Mine experienced a high wall slide in the DII Area at the west end of the pit. #34210 loader was cleaning coal at the locations and was trapped by the wet soil. A backhoe was called to the area and the loader was pulled out of the slide material at 12:47 pm. No one was injured and there was no equipment damage. MSHA hotline was called at 12:48 pm.
Rock fall in the face at 2 heading. The rock was 15' high, 8' wide and 40' long. Fall is dangered off and supported as per roof control plan. The fall will not be cleaned up.
Fall was discovered on 2 west submains, #6 entry, cross cut #18 at survey station #9967. Fall did not affect ventilation or passage of men. No injuries or equipment were involved. Fall did not occur in active workings.
A fire occurred on 54-BC-01 and associated transition chute to shacker conveyor (alternate fuel tire fluff feed system). Maintenance was modifiying a guard that the MSHA inspector had issued a citation on earlier in the day. It appears that some hot slag or sparks ignited some of the fluff which remained in the chute area. Fire was put out by Kern County Fire Dept.
At approximently 12:35 pm on 3-26-09 a roof fall occurred on the #1 working section in the last open crosscut.
Roof fall in the #2 entry at Spad #4926. Fall was 20'x40'x5'. No injuries, blocked passage or equipment involved. Fall was 2500' from an active section.

The roof fall was discovered on 4 Mains, 6 block, #5 heading. The fall was 3000 ft. outby the loading point, and measured 17' x 19' x 12'. The intersection was supported with 6 ft. Super Twist Bolts and 12 ft. Post Tension Cable Bolts. The fall occurred above the anchorage. No ventilation or travelways were affected.
As afternoon crew arrived on 02 section, they discovered a roof fall in #1 Entry inby loading point. Area was cribbed, timbered and dangered off. Area was investigated by State & Federal. 2 blks inby Spad 5836.
A fall has occurred 30' inby survey spad 1787 in the #1 entry of the No. 2 Southwest mains. Fall is approx 50' long X 20' wide X 5' high. Area has been timbered and dangered off. No affect to ventilation.
Fall discovered 3/24/09 at 1:40 pm. #5 belt, #2 entry, spad 17688, in the return. Fall was 30' x 20' x 8010' high. There was no equipment involved, no ventilation affected & no personal injury, however, passage of people was affected. MSHA & OMSL were notified and MSHA inspected. Area was breakered off and will be rehabilitated.
The top of a power pole owned by Allegheny Power supplying our 138,000 volt feed caught on fire and fell to the ground. This caught some brush on their right of way catch fire. The fire burned across onto our permitted property. Poles were inspected by Allegheny Power and they cound find no problem.
Continuous miner was in the #6 entry and was mining cut #30 when a rock approximately 7 feet thick x 18 feet wide x an undetermined length fell on the continuous miner. The continuous miner was unable to tram out from underneath the rock and a retriever was utilized to pull the miner out from under the rock.
A fall of roof occurred in the 1st Left panel off the 1st Right at 13+70, entry # 0. The fall was approximately 30 feet in length x 20 feet in width x 10 feet thick. The area was supported with 5 foot fully grouted roof bolts.

A fall of roof occurred in the 1st Left panel off the 1st Right at 17+30, entry # 0. The fall was approximately 30 feet in length x 20 feet in width x 10 feet thick. The area was supported with 5 foot fully grouted roof bolts.
A roof fall 40' L X 19' W X 7' H was found at 8:00 AM at 9 #4 butt station 4627 X cut 3-4. The fall does ot impede ventilation or travel.

Roof fall in #3 Section at Spad 33289 - 33285. Fall was in worked out area. 20' wide, 70' long, 6' high. 5' glue bolt was used. Entrance to fall is dangered off and supported as per Roof Control Plan.
Roof fall was found by JD Adams it is on the North east mains at Bk-48 and is appr. 6 high and goes from number 1 entry to number 7 it is in aircourse that is required to be checked weekly. It's not close to any working sections.
At approximately 5:30 am on March 22nd 2009 a roof fall occurred on the # 1 section in the # 9 entry in the last open crosscut.
Unintentional fall of mine roof located in the Intake entry at 97 X-cut, in the North Main on the right side. The fall is approx 45' long, 20' wide and 10' high. Distance from the active face is 4,000 feet. Ventilation to the section is uninterrupted. All the EE's have been instructed to use #2 primary escapeway. The Foreman and crews have traveled this escapeway.
East section, No. 5 Entry (roadway) 80 ft. inby survey station 12968. The fall measured approx. 80ft long x 20ft wide x 5 ft high. Five ft 3/4"" dia. Fully grouted resin roof bolts were in use as primary support.
An unintentional roof fall occured at #71 cross cut on the Main North Entry #3. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 20 feet x 20 feet x 6 feet. The area had been roof bolted with 6 ft. fully grouted roof bolts.
On call safety pro received call from Tyrone Town Site Volunteer Fire Depart that a wildland fire was appoarching our property at 4:22. At 4:45 pm guard shack radioed that fire was not burning on our land. At 5:02 pm another EE called MSHA to notify them of fire and at 5:07 called the state mine inspector. The fire was called out at 5:57 pm.
Roof fall approximately 30 ft long x 15 ft +- high in the intersection. No ventilation restrictions or injuries occurred.
A methane gas ignition has occurred on the mno#3 section roof bolter. The ignition occurred in the mine roof bolt hole. (Corrected copy from previous submitted form.)
A roof fall occurred in the 7th sub-panel north XC-24 off of the 8th panel north (spad # 8 + 80) old blet entry in worked out panel approx 8' high X 21' wide X 30' long. There were no injuries, witnesses or disruption to ventilation.
Roof Fall occured at spad # 365. Top was broken and laminated. Had been strapped.
While making a weekly return, inspection, the roof fall was discovered in the No. 6 Entry of No. 3 S.E. Mains at SS 1150 3000' outby the active mining section. The area where the fall occurred had previously been dangered off and all entries blocked off leading to it. It doesn't interfere with the ventilation.
The roof fall is in 1 entry of 2E about 36' inby XC3. This area was supported with 7' Install III bolts with timbers along each rib. The roof that fell is laminated volcanic sill rock. The fall is 25'x15'x 2 to 7' high. The cause is the composition of the roof in this area.
An alleged ignition occurred at the long wall face at or near shield #78 while mining coal. The ignition was described as a flash of orange around the shearer drum that lasted only about 2 seconds. The shearer was coming from the tail gate to the headgate. Power was immediately shut off. Methane readings were taken after, the readings were 0.01% with a hand held detector.
A roof fall occurred at crosscut 19 between #5 and #6 entries. The fall measured 5'H x 20'L x 19'W. There were no injuries but a stopping was destroyed interupting ventilation. The area had been bolted with 4' bolts.
A roof fall was discovered by the examiner approximately 20' long, by 15' wide by 5' thick. The fall will not be cleaned up.
We had the cap rock come down. It was in the tailgate of the longwall. It should be noted that the fall involved the skin only and nothing above the anchorage point.
An unexpected roof fall was discovered during a pre-shift inspection of the underground mine on the 2-10pm shift. The roof fall is above our normal bolting horizon of 8 ft. The roof fall did not cause any personal injuries, entrapment of an individual, impair ventilation or evacuation escapeways. No mining equip., infrastructure or mining utilities were effected by the roof fall.
Continuous miner was cutting in the #4 entry in left side of pillar, when the rib rolled off and went into deck of the miner. Rib roll pushed E-stop in and miner wouldn't start back. The roof bolter was being utilized to bolt up to the miner, when a rock 3' thick fell on the head of the miner. A retriever was utilized to pull the miner out.
Roof fall was found at break 19 while traveling on 2 West roadway. Roof fall was located in the break and did not affect the intersection or travelway.
A tailgate blockage was discovered on the 3rd left tailgate (alternate escapeway)
A roof fall was discovered on the 9E Section #2 Entry #3 Break Intersection. Roof fall was 25'Lx19'Wx8'H. The area has been breakered off.
Case# 1-61706518. The computer program that controls the automatic operation of the hoist was updated with the wrong version causing an error. MSHA was notified at 3:24 pm that the hoist was down. The computer error was discovered at 3:29 pm and the hoist was kept on standby. The hoist could be operated in a manual mode the entire time.
A roof fall has occurred on the 001-0 MMU, 1 break inby spad #601. The fall measured 19' wide x 40' long x 5' thick. Persons, ventilation nor equipment were effected. This fall area will not be rehabilitated.
The roof fall was dicovered by MSHA inspector during his weekly exam the fall was a continuation of an existing fall that had occured some time ago. The fall will be re-supported and cleaned up to extablish a travel way. The area around the fall will be dangered off to prevent access to the fall area 7 head at spad no 30063.
A distortion was found in the hoist rope. The hoist was taken out of service, 270 feet of rope was cut off and a new rope socket was installed. The car was tested and returned to service at 3:00 PM. No escapeways were affected.
A roof fall measuring approximately 15'L x 12' W x 4' H fell at the bottom of the slope.
West Mains 11 entry, 29 block, MMU 002, 3-way intersection. 20' long x 20' wide x 8' high. Support: 6' 3/4"" resin bolts. Fall dangered off, additional roof support installed around fall.
Non-intentional, non-injury roof fall occurred in the #1 entry 90' inby spad 1815 in the intersection. Fall measured 19' wide by 19' long x 7' - 8' high. Ventilation was not affected. Fall will be cleaned up and area supported by cribs.
The service hoist #4703 tripped out. Electricians conducted an investigation and found four blown fuses which were replaced. The hoist did not come back on line. The equipment manufacturer was called to site. The technical representative identified faulty SCR relays and replaced them.
A roof fall occured at the 1550 level in a small drift between the 1550 shotcrete plant, and the 1550 electrical shop. The fall was approximately 10'X8' by 24"" in depth. It did not pull beyond the point of anchorage, but did impede passage. MSHA was notified within 15 minutes of the incident.
While brushing the top with the continous miner head an ignition of coal dust occurred. This was of short duration of 2-3 seconds. No individuals were injured or no equipment damaged. Section was immediately shut down and MSHA notified and an investigation was made of the ignition.
Unintentional roof fall discovered this date. Area previously mined on or about 10/18/06 and was supported with 6' full grout resin bolts. Fall measured approximately 18'W X 75'L X 6'H. Fall location is 15 Zone, 11 block, Xct 8-9 & 9 Intersection 2 blocks inby station 2490. Overburden is at or about 280'.
Stockpile of coal located in maintenance yard for a long period of time eventually became hot on the southeast side of the pile. The area, although small in comparison to the over all size of the pile, will be isolated by removing non-hot material, then removing the hot coal.
This is to comply with violation #8075775 at the Josephine #3 Tunnel Mine. On 3/15/09 EE, 3rd shift Fire Boss, found loose rashy material had fallen from the highwall on the inby side of the fan house. The material measured approx. 9'H X 4'-6'L X 9'W.
A roof fall measuring 20' x 20' x 9' occurred near spad No. 4333 on the Jenny Branch section.
A roof fall occured in the #9 seam in the 2 East Panel in the #7 entry in the intersection of crosscut #28. The fall measured 20'x18'x5'. The areas was supported with 5' fully grouted bolts.
Hoist went down at 1530 PM. Called MSHA at 1600 PM 1-800-746-1553 to report it. AC breaker not staying closed due to faulty output from sensor unit, resulting in computer not releasing hoist brake. Notified MSHA at 1710 PM that hoist was back in service. First Report to MSHA was #1-61621341; Second Report to MSHA was #1-61621363.
An unintentional roof fall occurred in the No. 6 entry one break outby spad No. 1892, 6000' outby the working section. The fall measured 40' long, 6' high and 19' wide. The faall did not affect the travel of men or the passage of air.
Mine Foreman and MSHA Inspector were traveling in Right Return & found roof fall. 1300 Feet from face on a different panel at spad #3107 between #4 and #5 entry. Approximate size of fall 6' height, 19' width, 25' length.
Unintentional roof fall was found while making rounds inby spad 2580, approx 4 X-cut.
Pillar #2 into 3rd cut between #11 and #12 crosscut, a piece of rock fell on the Joy continous miner. No ventilation was affected, the fall was approx. 4'x10'x15' to heavy for the miner to back up with, the rock was pulled off the miner with a retreaver jack.
Wood chips/dust burned on the conveying area of the wood chip 'Wood Hog"" system. The fire lasted approximately five to eight minutes. The extent of the damage was a piece of burnt rubber skirting. There were no injuries related to this incident. Three ABC fire extinguishers were used extinguish the fire.
Lowering an LHD bucket down the shaft, slung beneath the skip. The bucket became lodged in the shaft and broke free from its slings. Damage to the shaft structure and skip guides has occured.
The elevator went to fault computer on the control screen making the elevator inoperative.
During the shift, while walking the Airways, mine foreman and another EE discovered 2 unplanned roof fall's on Tom's Branch. The falls are located in the Return Airways near spads 598 and 604.
Making Weekly examination of Right Return and at 2:30 pm found roof fall at spad # 2980. Fall was 3200 feet from the Face and approximate size of fall was 10' height 20' width 30' length.
Non-intentional, non-injury roof fall was discovered in the Northwest Mains, 90' inby SS# 3040 in the #4 entry. SIze of the fall was 16' wide x 12' long x 6' thick. Fall will be cleaned up and the top will be resupported.
A roof fall 30' long, 20' wide, 5.5' thick was discovered in the return from the abandoned 8 mains. The fall was 4800' from the working section. It occurred due to laminated coal and rock above the 5' torque tension roof bolts. It did not impead travel or ventilation, no injuries resulted. Supplemental support was installed at Fall entraces and barricaded.
An un-intentional roof fall occurred in the LOB intersection of #5 face & LOB left. The LOB spad # was 1925 and this was 2 crosscuts inby the feeder located in the #4 entry @ spad 1909. The area was supported with 4' resin bolts, and 6' torque tension bolts. The fall measured approximately 20' wide x 30' long x 6' thick. It will not be cleaned up.
Left rib from sill to spring line slid in the 3575 HGB, causing less than four feet of ground to be exposed.
An unknown electrical problem rendered the Production hoist inoperable. There were no persons underground at the time. Trouble-shooting revealed a contactor linkage had gone out of adjustment; back in operation at 12:45pm.
The roof fall was discovered by MSHA inspector during his weekly check the fall was a continuation of an old fall that had occured when the longwall panel was mined. The area will be dangered off and not be cleaned up. Headgate spanel at cross cut 19 thru.
An unknown electrical problem caused the Koepe access cage to become inoperable. Mine evacuated; no injuries. Troubleshooting into mid-day revealed brake caliper switch had gone bad; back in service at 1:00 pm.
An unplanned roof fall occurred in the #2 Gas Mine at spad # 445, break # 35 that measured approximately 90' long x 18' wide x 15' high. No injuries were reported. The area was developed on 2-6-08.
An unplanned roof fall occured at break 118 on 12 Headgate in the crosscut between entried 1 and 2. The fall measured 20' wide x 60' long x 6' height. The fall did not affect ventilation, equipment or passage of persons.
A rock fall occured at approxiamately 3:30 am in the Main decline approximately 300 feet from portal. The fall was estimated at 8' height by 8 foot wide by 25 feet long from the rib and back. The size of the fall has continued to grow as miners work on installing additional support required by MSHA to access the fall area.
Rock fall occurred in the #1 entry at break #30 and extending to break #31 in the 2nd left panel off the 1st left submains off the 2nd left panel off the 2 West mains. The rock fall was approximately 18' wide x 70' long x 5'-6' high.
An Unintentional roof fall was discovered ~8:30 AM in the South Mains Primary Escapeway. The fall was located between #103 and #104 crosscuts, # 7 entry and measured ~ 50' x 20' x 5'. The area was supported with 48"" fully grouted resin rods.
12"" waterline broke impeded the escapeway, miners inby were withdrawn until the primary escapeway has been pumped of water.
Foreman making weekly examination found rock fall at break 130 in #7 entry on intake. Approximate size of fall: 5' height, 19' width, 40' length. Fall was found at 2:00 pm and was called in to MSHA as soon as foreman got outside.
A roof fall was discovered at 3 main 22x-cut. 6 to 7 x-cut only. Fell above anchorage 10 feet wide 30 feet long. No men or machines were injured or trapped, no ventilation or travel was affected. Six foot fully grouted resin bolts and 12 foot post tension cable bolts were in use.
During a check, the hoist would not operate utilizing the automatic controls. The hoist could have been operated on hand control, but miners were evacuated until the automatic controls were operational.
A fall of the roof occured on the south east life of mine mains parellels at crosscut # 31 #7 Entry. Ventilation, personnell nor equipment were involved or effected.
Damage to hoisting equipment occurred while moving equipment on C Shift. The main power on the A Shaft Man Hoist knocked and would not reset. The main breaker had to be changed out. The elevator was down for a period of time longer than 30 minutes.
A roof fall measuring approximately 80' x 20' x 10' occurred on the Jenny Branch section between the No. 6 - No. 7 crosscut.
We had a rock fall on the #5 beltline. This is a non-producing mine. All equipment has been removed from the mine. Fall was approx 100 ft long, 5 to 6 foot high. All approaches have been timbered and dangered off. We will not be cleaning up this fall due to the mine being closed.
It was reported that a face ignition ocurred in the # 3 face of the 4B section.
An unitentional roof fall occurred at 11 x-cut beteween the track and return in the 12I section. The fall measured approx. 170ft long by 16ft wide and 18ft high. *** no injuries or equipment damage at this time
A roof fall measuring approx. 20'Wx50'Lx10'H fell at 10:53 am inby spad number 34187. Both state and federal agencies were notified.
The Crabapple elevator was taken out of services for more than 30 min. to replace computer drive component. This was a planned outage, no one was effected by this outage.
After the fall occurred there was evidence of water in the upper layers of the rock strata that may have contributed to the failure.
Elevator went down for a door switch at approximately 8:10AM. No one was on the elevator at the time. Everyone was notified of thge problem. Production was stopped and the people were removed back to he loading areas. Elevator door switch was repaired and the elevator was operating correcttly at 10:30AM.
Had a portion of roof fall in #3 entry while mining. Roof contained surface cracks which were not visible due to mixed laminated top which was stuck to sandstone roof. Rock was approx. 3' to 4' in width, 3' to 4' in thickness and approx. 27' in length.
The elevator stopped running and the part that was changed out (door switch) was not the problem and by that time that occurred it exceeded 30 minutes. Cause of this interruption was a slack rope governor switch adjustment. After the governor switch was adjusted the elevator was run empty and worked properly.
The Bailey hoist was inoperable for over 30 min which interfered with the use of the equipment. It was found that the ""call"" control button on the bottom was stuck in. No injury or equipment damage to report.
A roof fall occurred in the #9 seam mine in the Main North at crosscut #21 in the intersection of #4 entry and continuing through the pillarline toward #5 entry measuring appoximately 45ft. long x 19.5ft. wide x 8ft to 10ft. high. This area was supported by 6ft. tension rebar bolts, 9ft. mega bolts and 12 ft. cable bolts.
While mining, loose roof rock broke a hose off of the left side spray bar and an ignition occurred from a methane bleeder. Mining was in the # 1 face of the G9 section.
An unintentional roof fall occurred on March 3, 2009. Fall occurred on the #3 belt, cross cut between # 5 to #6 entry, break #2, Spad No. 1225. No injuries occurred from the fall. (6ft. torque tension bolts)
Roof fall A - F Entries at 19+86. 18' wide, 20' long * 7' high in 5 intersections in the 3 south non-working section.
A roof fall occurred in the #11 seam mine in the 2 Main North at crosscut #26 in the intersection of #10 entry measuring approximately 25ft. long x 18ft. wide x 5ft. to 7ft. high. This area was supported with 30 inch fully grouted roof bolts.
As the shear was cutting out on the tail side of longwall a small flame was osberved and was extinguished within 2 minutes using a wash down hose.
A roof fall occurred on #4 unit, crosscut 14, #5 entry. The fall measures approximately 16' long, 16' wide and 5' high. The area is supported with 5' tension rebar bolts and 12' cable bolts.
A fire started in the idler repair shop around 6:45 p.m. after the employees had gone home for the day. No injuries occurred. The fire destroyed most of the building and burned for more than 30 minutes making it immediately reportable to MSHA. No obvious ignition source was found.
#4 entry, x-cut 36, #4 unit. Length 30' x width 18' x thickness 5.
MSHA inspector was making a routine inspection and came upon 2 falls in the old 1st right headings one break inby spad # 657, one in intersection of #3 entry and one in intersection of #5 entry. The fall in #5 measured approx. 20 x 50 x 8. The fall in # 3 measured approx. 20 x 30 x 8.
A roof fall in the #5 entry at the face of the first panel of #4 belt. It was 5 to 7 feet thick 20' wide and about 35 to 45' long. It was a mine fault.
An unintentional roof fall was discovered in the No. 9 return entry. The fall was near spad no. 3952 and was approximately 18' wide x 20' long by 4' thick. The area had been previously bolted with 42"" fully grouted bolts. The fall will not be cleaned up as the return was routed to the adjacent entry.
A roof fall occurred in the #9 seam in the East Mains in part of the intersection of crosscut #30 and extending inby the pillarline toward crosscut #31 measuring approximately 30ft. to 35ft. long x 18ft. wide x 9ft. to 11ft. high. This area was supported with 9ft. mega bolts.
An unintentional roof fall occurred on the 1 south track haulage. The roof fall measured approx. 200 ft long by 16 ft wide by 12 ft high. **** No injuries to report as a result of incident***
Elevator travel switch was out of time. Elevator would not come down. No one was on the elevator at the time of the malfunction. Elevator down time: 180 minutes.
In the process of chipping ice in the Livingston Portal elevator shaft, a piece of ice fell damaging an elevator door switch on the shaft bottom. An employee became stuck on the elevator in the process. The employee was able to get off elevator at 7:48PM. There were no injuries as a result of this incident.
Employee got out of cab and stepped toward window & knee gave out. Employee did not slip or trip, footing was good, maintained 3 point contact, from a flat surface, but as employee shifted his weight his knee gave out.
A dragline oiler had been walking the 1570 dragline. He was last seen near the end of a walkway and was later found between the shoe and the revolving frame. There were no witnesses to the event. The employee was fatally injured.
Employee was reaching for something and his hand was caught in part of the machinery.
While drilling a water well, the injured floor hand was making a drillpipe connection. The injured employee did not wait for the other floor hand to get tongs completely around the pipe, he put his hand in a pinch point, the tongs were then latched. Injured employee had the tip of his left ring finger amputated.
Crew was pulling 670 feet of rail outby with 5/6 nylon rope and clevis. Clevis bent and pin broke striking employee's left arm and rib area.
Employee was bolting top when a piece of draw rock fell out between roof bolts and struck employee on the back. Rock measured approx. 3' x 5' x 1-8"".
EE placed his right hand on the bolter and a rock fell, striking his hand. He was air lifted to St. Louis Des Peres Hospital. Lost time injury.
Employee states that while he was helping another employee to to move some hydraulic jacks around on a pallet for shipment, he was squatting down and felt pain in lower back.
An employee was working at the Soda Ash Product Load out area. While uncoupling the loaded car to spot the empty car the employee fell on the tracks. He was moving to get off the tracks when his foot was caught under the wheel of the car and sustained a crushing injury to his left foot when the front wheel of an empty railcar rolled onto and over his foot. .
Employee was hand mining and scaled a piece of loose down from high and hit in basket on top of right foot. He was taken to company dr. After continuous pain, he was taken back to company dr and was taken off work on 4/8/09 making this reportable.
Operating a line small pain in hand after lunch went to B line about 1 hour stopped to dump bags and saw hand was swollen.
The man was walking towards the continuous miner when he stepped in a hole and twisted his knee.
Crew engaged in line cleaning operation. Pressure dump device was operated by co-worker (procedure violation). Work activity was suspended to verify process issue, EE placed L hand over nozzle to block low pressure water bleeding from nozzle. Co-worker asked if he was ready & mistook no for go. High pressure water resulted in laceration. Surgery initially proposed & declined.
Employee was advancing a piece of roof mesh in 3 north right section and stepped on alump of coal rolling his right ankle. He began missing work due to the injury on 4/6/09.
The employee was bending an 8 foot bolt during installation when his right hand slipped from the bolt. This caused his left wrist to twist in a downward direction. This caused the bone in his left wrist to chip.
HE WAS REACHING FOR A PIN, WHEN HE FELT SOMETHING POP IN HIS SHOULDER **STARTED MISSING ON 05/12/09**
The employee was tramming scoop from outside to face. Got his light cord hung in roof bolt or plate jerking head around.
While working as a Greaser, tossed brake drum and something flew into his right eye.
On 3/31/09 the employee was kneeling down using a water hose to clean material from under a belt pan. The hose blew out, causing the employee to be startled and strained his left hamstring in the process of getting out of the way. The employee started missing work on 4/2/09 for a severe left hamstring strain.
Pulling roller, tripped, causing him to fall into tailpiece.
On 01/05/09, 5:30am ee was using a poke bar to loosen frozen material in rock hoppers. During his shift he felt discomfort in his Lt. arm, felt it was jarred while poking frozen material - he has been treating with Physical Therapy - further testing has indicated a torn rotator cuff in Lt. shoulder. Employee has become a lost time as of 03/31/2009 pending surgery.
Member was tramming shuttle car from #3 right crosscut toward #4 entry. As he was making the right hand turn the shuttle car struck the inby corner of #3 coal pillar causing a whiplash type injury to his head, neck, back & legs.
Person was walking around the mantrip when he stepped into a rut in the roadway causing him to slip injurying his knee.
Employee stepped in a hole and twisted his leg & hip.
EE attempted to pull rubber strip that covered rail car scales, when he felt a strain in his abdomen. Physican diagnosed as hernia, surgery was required to correct.
Employee was trying to hang the continuous miner cable by himself when he felt pain in his right shoulder. The pain continued to get worse and he had to have surgery on it on 5/11/09. Ron worked until the day he had surgery.
Employee was roof bolting. When he was hanging curtain board he dropped curtain board. It hit his right thumb pulling muscles in it.
EE was operating scoop, when he ran into shuttle car, it strained his neck.
Employee was working on #3A Head. When he moved a guard it fell striking his left hand middle finger.
Injured got on top of flat car to move pallet and his right foot got caught on sheets of tin. He stated he felt pain in right knee. He finished the shift and went to a doctor the next day. He is being treated for pain in right knee.
Employee was bolting top, was swinging in boom to put up inside bolt and felt something pull in right shoulder.
On the above date, the injured was shoveling coal in the clean coal tunnel when he felt a sharp pain his left hip and lower back. After being evaluated by a doctor it was found that he had a degenerative disc issue and until the date of April, 19th of 2010 he had resumed work until the old injury botherd him enough to seek further medical assistance. And be off from work.
He was exiting a truck when he slipped on the running board and fell to the ground causing a Meniscus tear to the right knee. He continued to work at his regular duties until 5/13/09 when he was removed for arthroscopic meniscus repair of the right knee.
Employee was pulling roof bolts off the rear of the roof bolting machine and twisted his back. No medical attention received at this time by employee. Several days later employee calls and reports broken bone in the left foot which occurred when he stepped off the rear of the machine to the ground. Report is being sent to show injury to left foot.
Employee was removing an old hooder above a box car and realized he was tied off to the hooder when it broke free knocking him off the top and sending him to the ground. He was able to disconect before he hit the ground. He sustained damage to his pelvis, right shoulder and left knee. No surgery or bracing of any kind was needed.
Employee was struck by the roof bolter cable when it got jerked down due to the reel being on while the bolter was backing out of the heading.
EE was stepping down from rubber tired dozer. Stepped on a rock or chunk of coal and twisted knee. Reported injury to employer on 04/09/2009. Arthroscopic surgery completed on 08/14/2009 with subsequent time away from work.
While attempting to place a roof bolt in drill head, employee inadvertently lowered the drill head onto his left foot.
Assisting in picking up a corner bit for a D11R to put in a loader bucket when it slid out and fell on his left foot.
While doing preshift duties, ee stepped in a hole. Later in the shift while shoveling coal, he felt a pain in his waist running down his leg. Pain was minor but noticeable. Did not miss any work until April 8th when pain became more intense. He went to doctor who advised an MRI and time off.
EE was to prevent entry into blasting area. Pickup used to block traffic moved too close to intersection of blasting area & main drift. Fly rock from blasting area traveled 500 ft. & struck employee sitting in pickup. EE was struck by small rock which fractured safety glasses causing glasses to penetrate eye. Evaluation ongoing. EE stated he drove in prior to shot's completion.
Employee was moving plywood and Iron wear bar to top of transfer house. 90 minutes after task completed, employee experienced pain in neck muscles. Sought medical treatment after shift. Diagnosed with severe cervical strain. Missed his first shift 04/03/2009.
EE was kneeling down on one knee putting his tools back into his tool box whem a shuttle car trammed past him and ran over his left leg.
EE was operating a scoop pulling belt. His hand was placed on the back of the scoops deck and bumped into the rib mashing two fingers.
Two employees were installing 8"" pipe. While tacking pipe in place, a spark entered a piece of PVC stub protruding from floor. This spark ignited the PVC glue gasses that were trapped in the piping system, creating an explosion that sent PVC pipe fragments throughout the plant. Two employees received medical treatment for shrapnel injuries.
Employee was tramming to miner through x-cut watching his cable and he thinks he ran over a rock bouncing up against the underside of the canopy striking head. Compliance with rules/regs not a factor. Mining equip/ systems a factor, protective equip. not a factor, Miner proficiency not a factor. Cause of the injury was safety awareness.
EE was profiling the 1 South belt structure using a chain hoist that was hooked into the chain that the belt was hanging from. The EE was standing on the belt which was 60"" off the bottom, when the chain hoist came loose, causing the EE to lose his balance and fall from the belt to the mine floor.
Employee was exiting his vehicle. The employee stepped out of his vehicle onto an icy patch that was snow covered. He subsequently slipped and fell and injured his left side ribs.
EE was cleaning out the leg pockets of the shields in preparation for extraction. As EE went to crawl out of the shield, he twisted his back.
DID NOT START LOSING TIME UNTIL 4/13/09. Operating a continuous miner. Roof broke loose and hit slider curtain and rail. Rail hit employee, knocking him to the ground.
miner was closing the engine door and caught his finger between the door and the exhaust.
Employee was driving wooden wedges under the tub of a 1370 dragline to tighten the cribbing under the tub. On his backswing, he hit some adjacent cribbing, altering the path of the sledge hammer and hit his left foot.
While setting a head drive employee stood up and felt a pain in left knee.
Employees was pushing spoil with a dozer. The employee went forward over the pile of dirt and when the dozer fell over the dirt the blade hit the ground, jarring the employee.
Operating the scoop from the opers. compartment with his right hand, looking toward the bucket, his left arm was in such a way that his hand was hanging out left side of the oper. compartment. When he started to turn the scoop he pinched the fingers of his left hand between the front stop block and the frame of the back half.
Employee stated he was carrying a 1/2 bag of rockdust walking backward dusting the face area. As he twisted around he stepped in an uneven place on the floor, stumbled and this caused his leg to buckle under him straining his knee. He was transported to Medical facility. Was taken off work on 03/31/2009.
EE was getting off of 773 rock truck. He fell from top step landing on his side (left). MET checked him out, he was taken to hospital and discharged later that night.
Injured employee set a roofbolter drill boom pot down on top of his left foot. He received a fracture to the little toe on his left foot and soft tissue damage to the top of his left foot.
Employee is a examiner and stated that while making his route which is walking he slipped going under an underpass and had pain in left and right ankle. Started missing work 6/6/09.
The employee was backing a load of supplies into a crosscut, while looking over his shoulder he ran into a pin board that was hanging down, jamming his neck. He didn't start missing work till Dec. 14, 2009.
Employee was stepping down from a bulldozer when he missed the bottom step, located on the pusharm, and pulled muscles in his lower back.
Taking 3rd cut fom #4 entry, squatted down, roof soap stone, rock fell from between rib and bolt striking employee's right shoulder area. Taken to ER for treatment, x-ray negative. Saw orthopedic 3-31-09, diagnosed with contusion to right rotator cuff and right collar bone.
Employee was pulling 3"" PVC pipe in No. 3 belt entry, pipe hung up and struck top of his right foot.
Employee got powder on wrist causing an allergic reaction, burn on right wrist.
Employee was coming down the steps to the parking lot. As he stepped to the ground, his left ankle rolled to the outside, causing a popping noise.
Injury went to the finish mill room to get a sample of cement from the number 2 finish mill and alleges that she tripped on the floor as she turned to walk back to the lab with the sample. She claims that she tried to catch herself with her left arm and as a result injured her left shoulder. This is a lost time accident.
Employee was attempting to sit down inside company vehicle and in the process, felt severe pain in his lower back. This resulted in a strained lower back and two lost time workdays as per doctor's advice.
Emp. was repairing BE 495II shovel. Metal 1"" thick, with the dimensions of 48"" long by 32"" tapering to 16"", was being used for the repairs. Metal shifted and slid off supporting tabs and beam. The plate fell and struck Emp's. left forearm and then his wrist. On 3/30/2009 Emp. was examined by NM Orthopedics and it has been determined he fractured his left wrist.
1"" air line caught on bucket lifting pump into air. The pump slid off and fell 15' hitting employee on collar bone and right ankle. Collar bone is broken and ankle is bruised.
A rib roll hit him. His right leg was hurting. Didn't see any injuries.
Employee was burned by hot product coming out of a port on the preheater tower. He received 2nd degree burns.
Employee began missing work because of surgery to his right knee.
Contract driller equipment dust box was not secured. When mast was lowered, the vibration caused the bottom portion of dust box to fall, striking miner on right shoulder.
The first Employee put a pallet onto the packing station using a forklift. The second employee was cleaning the rails on the packer. The second employee was between the pallet and the rail. As the first employee was bringing the pallet, he did not raise it high enough, bumping into the empty pallet and pushing into the second employees feet and pinning his feet against the rail.
This report is being filed late due to first believing this event to be sports related. It is now believed this could have been work related. He states that he became entangled in a roof bolt cable, lost his balance and fell to the mine floor.
Injured at the 8E switch of 8HLG track on his hands and knees pushing a 120' piece of sch 11 pipe over with his arms. When he started to stand up he felt a pop between his shoulders causing pain. He was transported to the hospital. He is being sent to a specialist to diagnose the injury.
Employee slipped on ice, in the parking area, injuring his right foot.
EE was helping take shuttle car to the section. He was turning a ride around to get out of the way of the shuttle car. When turning the ride, his head jammed into the mine roof. EE stopped what he was doing and stated his neck was hurting.
While lining up pins for a stiff arm on a CAT D-10 dozer, the pry bar being used slipped and the miner fell foward striking his right ear and right jawbone on truck outrigger. Miner was given an icepak and said he would continue working. Miner did not seek medical attention until MAY 02, 2009. Miner did not miss any work between March 25, 2009 thru May 05, 2009.
Employee was pulling cable with a battery powered personnel carrier, ran over something and his head contacted the roof resulting in a neck injury.
Employee was helping to change overhead belt on the screening plant. When the clamp was released, it hit employee in the arm and cut his wrist. Employee received stitches and was released back to work no restrictions. Employee had some pain and bleeding overnight and went back to the doctor on 3/26/2009. Wound was cleaned and dressed, released back to work no restrictions.
Employee stated he was removing a belt ""A"" frame from mine roof. One end of belt ""A"" frame dropped down. Other end caught left hand ring finger between roof and ""A"" frame causing laceration to finger requiring 17 stitches.
Employee was in Scale office before shift. Stood up from chair to walk to fax/copier, took one step and knee gave out. (Did not hit anything, twist or turn). Employee said she had been exercising a lot using ""WII Fitness"" program.
Truck stalled and drifted back onto berm and turned on the side.
Injured and coworker were moving an ""I"" beam across the track. The beam flange caught on the rail and swung around and struck injured on his right shoulder and arm.
Employee was putting drive line bolts back in a 980G loader. The rachet slipped off the bolt head, striking the employee on the left side of his nose and corner of his left eye. (stitches)
Injured was in the process of shutting of a discharge waterline valve and was struck by block from a pier that was used to support a stopping. The pier came loose from the stopping and fell over. The block struck his right knee and right calf. His right knee was strained.
He tripped and fell, twisting his left knee.
EE was lifting slabs of sandstone to place them on a pallet, when he sustained pain to his lower back. Physician diagnosis: Low back sprain/strain.
Miner was walking across metal grating slipped and twisted knee. Miner did not begin missing work until 03/31/09.
Driver was traveling down ramp reforming to shovel when he made contact with highwall with right front tire of truck overturning on left side.
Stepped on loose ladder on left rear of the machine and fell to the ground.
The employee was welding an ibeam the steel i beam was rolled over to access the other side, the beam came loose from a chain and fell, about 2 feet onto his left foot. His left big toe was fractured behind the steel toe in his boot. This resulted in a lost time injury
EE stated that he was drilling a hole to put a board against the mine roof, when the rock binder rolled out, causing the vetilation tubing to fall and strike the injured EE and knocked him to the mine floor. EE suffered bruised ribs as a result of the incident.
Employee was replacing bolts in gallery structure. Driving old 3/4"" bolts out of wood structure using new 3/4"" bolts as a punch. As he was striking bolt with sledge hammer, his left elbow struck an existing bolt. Bruised left elbow.

Miner picked up a piece of ventilation tubing and strained his back.
Injured was standing on the side of a conveyor belt tail piece reaching for some shear pins that were on top of a belt feeder. His foot slipped which caused the employee to loose his balance and fall. He landed on the side of the tail piece of his lower back resulting in a contusion. He visited his family dr on 3/30/09 and was removed from work.
Injured stated that as he started to tram a 2-man Johnson Industries battery ride, it went into high tram unexpectedly. Bounced over a few ruts in roadway and was jarred off ride. Landed on right lower pelvic area.
The EE was carrying a welder in his hands, when he felt a sharp pain in his lower and middle back. The EE id not finish his shift.
EE WAS SQUEEZEING THE BELT CLAMP TOGETHER AND FELT A PAIN IN HIS RIGHT SHOULDER. HE HAS WORKED WITH THIS TILL NOW AND WILL UNDERGO SURGERY TO CORRECT THIS.
EE was removing electrical cabinets when he came into contact with electrical current. He received an electrical shock.
Loading high line sled in scoop bucket, pushed ram jack out and pinched hand between sled bucket.
Injured was struck on the right foot by a rail that a scoop hit when tramming in the injured work area. Injured was transported to the hospital where he was diagnosed with a broken right big toe.
Employee was walking along side of the F-17 belt storage unit and slipped in mud and twisted her left knee. Employee began missing work due to the injury on 3/30/09.
While the employee was changing a trapping shoe on the shear with a hammer, he felt pain in his left shoulder. He has been seeking medical attention off and on since the injury. It was determined to be a rotator cuff tear on October 9th and he received surgery on October 29th.
EE went to the section power center to re-set the #3 continuous miner breaker. The breaker went phase to phase and flash burned the back of his fingers on his left hand. This is a lost time accident.
The employee was driving a 777 truck at the Preparation Plant when his incident occurred. The truck shifted hard causing the operator to jar his lower back. Employee began losing time as of 03/26/09.
EE and another co-worker were moving the miner cable out of the roadway at Bk 11 on 8 belt in the track entry when coal rib rolled off, striking him on the left ankle, causing a bruning sensation. He was taken to the hospital where no fractures were found and he was given light duty work for 10 days. He had no laceration or markings on his ankle.
Injured was walking and stepped in a rut straining right calf muscle.
Employee was backing up on a forklift going about 2 to 3 MPH, employee seen another forklift and stopped suddenly, he felt a pop in his neck.
While cleaning up maintenance yard, Injury lifted a piece of metal and felt pain in his groin area. No medical treatment or missed time. It was reported as an incident only. However, it appears that a hernia has resulted in this strain and he just sought medical treatment on 10/3/09.
Injured was walking the primary escapeway. When he was going across the second overcast near the Y he said he caught his right foot on the platform and fell forward. When he stood up he felt pain in his right foot. He finished the shift and came to work the next day. He was transported to the hospital sent to another hospital for treatment of a sprained right foot.
The employee was helping the miner move to the next cut and a rock fell from the mine roof striking the employee in the head. The piece of mine roof measured app. 4"" x 10"" x 12"".
Employee was hanging miner cable and strained lower back.
Injured ee was cleaning gob off the track and twisted his knee around backwards.
While installing a roof bolt the resin cartridge burst and resin came in contact with the employees right wrist. A rash formed and the area became infected.
EE reported: When he started to bend over to pick up a roof bolt supply, he experienced severe pain to his low back. There was no event that caused this pain. He was bending at the hips when he experienced the pain. He was transported to a medical facility.The x-rays were negative & he was given a prescription for pain meds. The mine is charging this as a work related illness.
Removing bottom roller from belt his helper bumped bottom roller causing finger to be pinched between roller and structure. Cause was lack of communication. Compliance with rules/regs not a factor. Mining equipment was not a factor, ppe was a factor, miner proficiency was not a factor.
Employee was trying to release bound cable on jumbo reel to set up drill. Jumbo reel turned while his arm was trying to release bound cable, causing a fracture to both bones in his forearm.
Employee was walking up a slight grade in the #2 entry, F-19 section and felt a pain in the back of his left knee. He continued to work and began missing on 7/16/09 for surgery.
The employee stated that as he was walking along D-belt, he was struck in the head by a piece of rock measuring 2'x5""x1"" inch. The incident resulted in lost time.
Rock lodged in crusher/feeder. Lock out/tag out procedures taken. Employee went into feeder to pry rock loose. Rock rolled onto employee's leg.
While building a brattice, employee felt a pain in his back.
US Transport, Inc lime delivery driver was unhooking a lime trailer discharge hose without completely depressurizing the trailer and the remaining pressure in the hose blew lime into his eyes. He received temporary lime burns to his eyes.
The shovel was loading a haul truck when it faulted out causing the bucket to contact the bed of the haul truck. The operator of the haul truck suffered neck pain and has been monitored by a physician while continuing to work. The employee underwent a series of tests on 05/21/09 and was diagnosed with 2 ruptured discs.
He was attempting to uncouple a diesel mantrip, when he lifted the bar he felt pain in his right shoulder. He continued to work at his regular duties but continued to have pain. He was removed from work on 4/23/2009 due to a possible left rotator cuff tear.
Employee went to start the outboard motor and it was in gear. To prevent cranking the motor in gear, the rope will not come out when the motor is in gear. He pulled hard on the rope and pulled a muscle in his lower back because the rope did not come out.
Injured was carrying a joint of 8"" waterline and stepped in a hole, twisting his right ankle. When he fell, the waterline landed on his right foot. X-rays negative. Injured given few days off work to heal.
Miner was using bottle jack to to lift wash plant screen box to change springs. As miner reached to place block under screen box, screen box slipped off jack pinching right thumb. Right forearm also broke.
While roof bolting, a piece of draw rock, approx. 8""x4'x4', fell between the bolts and struck injury on the left shoulder. The wittness to the accident saw the draw rock give way and told him to run, had he stayed under the protection of the ATRS, he would not have been injured. He sustained a bruise to his shoulder and return to full duty on Monday.
EE stated that he was walking around the boom of a shuttle car, when he stepped in a rut in the roadway and twisted his right ankle, resulting in a fracture. The rut measured approx 4"" in depth, no loose material was observed in the area of the incident. Injured EE then walked inby to 39 crosscut where he received med attention. Transported to hospital.
Victim was using a torch to remove a valve bank from a roof bolter. The torch accidentally contacted an unseen hydraulic hose and caused oil to spray out and ignite burning his left arm and face.
Dozer fell off rock and arm rest on seat went under his ribs as the dozer hit the ground.
EE was using the sucker truck to pump silt out of the #6 sump when the suction hose surged and the EE felt a sharp pain in his right shoulder. EE continued to work and visit with his Dr. until 11/22/09 at which time he had surgery on his shoulder.
Painters were spraying epoxy, they wore respirators, in general area but not the same room. Work area was unventilated, fumes gathered at ceiling elevation. Employee was trimming out the fire sprinkler system at ceiling elevation. He was overcome by fumes, fell off ladder.
Injured was removing belt structure when he felt pain in left shoulder.
Loader/scale operator running loader experienced pain in lower back. Did not report to office untill 3/30/2009. Went to doctor on 3/31/09, was released back to work with a 25 lb weight restriction.
A drill steel bowed, operator spun steel and hit his left hand.
Employee was loading supplies on to his roof bolter when a rock fell from between bolts and struck him in the lower back.
We were marking off belt line for the longwall belt. We had to use a 12' ladder due to the height of the mine roof, while moving the ladder under some cables to reposition it the ladder slipped. While I was still holding onto ladder. Tearing rotary cup in my right shoulder.
Employee was exiting his personal vehicle, standing by car door, reached in to grab lunch box from car seat and his right knee popped. resulting in a right knee acl tear.
Employee was doing a pre-op check of a D11 Cat dozer. He openned the door and stepped out of the dozer to check the hydraulic oil. As he was bending over, he felt a pain in his lower back. The pain got worse as time went on and he asked to have a doctor look at it. Lower back strain.
While descending from the tip of the continuous miner, the employee slipped and fell, striking his back on the mine floor. He began missing work on 4/01/2009 and the injury was confirmed as work related on 4/6/2009.
EE was in the process of changing a flat tire on a 3-wheeler transport ride underground on #2 unit. He had hooked a chain to the front of the 3-wheeler and was in the process of raising the ride about a foot and a half, when the chain came off, causing the ride to hit him on the left knee. Because of low coal, he was on both knees. He was taken to a local hospital for cut to knee.
Unknown shoulder strain did not require medical attention at the time but since then has had to have surgery on the shoulder on 8/21/09.
Moving a rock to its side and pinched his left testical.
Return idler was on the ground in front of the Maintenance shop. Employee was distracted and tripped over the return idler. Stitches in right leg.
He was performing torque test on bolter with torque wrench, the torque wrench came free and struck him in the left groin area. He was removed from work for surgery to the left groin.
EE was walking down the belt, when he slipped in loose coal and twisted his left knee. He became a reportable lost time injury on 4/23/09 due to surgery. He worked up until his surgery date. Reportable lost time 4/23/09 due to surgery.
EE was making his normal rounds when he noticed a crane operater placing an NKM screen into a staging rack. EE noticed that the crane operator was having difficulty, he went to help the operator land the screen (normally a maintenance job). While helping, he placed his hand in a potential pinch point and when the screen was lowered, it pinched his two middle fingers.
Dozer was tramming out of shot. Got to the bottom of shot and dropped over a ledge approx 4 feet in height. Operator had his seat belt on. Said it twisted him.
The employee stated that he experienced pain in his back after lifting on the tongue of an air compressor.
Employee had connected diengaged car puller to C-6 rail car, then proceeded to push cars with track mobile, car puller became engaged causing cable to stop spolling. Cable became tight then causing cable to become disconnected from clamps, hitting employee on elbow.
Employee ALLEGED to have felt pain in his lower back and both legs while unloading three (3) Heintzmann jacks off a battery powered ride. Note: Employee worked 3-23-09 & then began missing.
While bringing down the strong back to pick up starter sheets he noticed one sheet was not on the hook. While he grabbed the copper bar and lifted it on the hook the strong back was still rocking causing his index finger to get caught between the strong back hook and the copper bar causing a circumfrential laceration to the right index finger.
The employee was working on a miner and was using a 3/4"" ratchet wrench and bar to break loose bolts. When the bolt broke loose suddenly, he fell and strained his neck and shoulder. He worked his regular job and did not miss any work until 4/13/09.
Lifting a cluth into position and trying to get a bolt started, felt something pull in shoulder.
Employee was lifting and pushing one end of the monorail and felt his right shoulder pop **EMPLOYEE DID NOT START LOSING TIME UNTIL 4/9/09**
Employee was stepping over track (18"") and tripped, when not raising foot high enough, fell hitting his knee/leg.
Employee was releasing load binder, when binder flew from chain, causing a fractured leg.
Injured was helping to carry a pump with a co-worker when he slipped on a rock causing pain to his lower back. He finished the shift and went to the doctor the next day. He is being treated for a strain to the lower back.
the man was installing a roof bolt near the rib. As he lower the head of the bolter, it struck his right thigh.
Employee was lifting a ladder back up and felt a pull and pain in his lower lumbar. Employee was referred to a specialist and has been off pending an MRI. Employee was referred to a Dr. after results of the MRI. Employee has three herniated discs in his lower lumbar and is now reportable due to not being released to return to work.
Employee had shoulder pain after grabbing a hand rail when he lost his balance while standing on a loader battery box. After a MRI was done, it was determined that damage was done around the shoulder rotary cuff. Employee was put on restricted work on April 8, 2009.
Employee was loading roof straps in the bed of a Ram car in 4 South mains. Using the ram to pull the straps in the bed, the end of the straps struck a piece of metal in the bed which caused the straps to swing hitting the employee in the lower leg.
Cleaning out secondary crusher, when cleared paddle turned pinching worker.
Employee was walking from clean side of bathhouse to dirty side and slipped while carrying his lunch box. He extended his right arm to catch himself and hurt his right shoulder.
Employee was trying to pull in the #1 shield from the #2 shield sims controls. The #1 shield had a chain hooked from the pontoons to the stageloader/face conveyor. As pressure was applied with the stageloader and the shield a connecting link on the chain failed and the employee was struck on the lower left leg just above the ankle by a section of the chain.
Employee was pulling a tire chock from a CAT 777 haul truck injuring his right shoulder. Employee did not miss any work time or been restricted as a result of this injury. Shoulder surgery was performed on 05/06/09.
Employee stated that he was grading the haul road, when the blade of the Grader struck a tree that was imbeded in the berm of the road way. This caused the grader to stall, resulting in a sudden stop, which caused pain to his lower back. EE refused medical treatment, but went to his family doctor after work and was placed off work.
Employee was pushing the outby trolley pole to disengage the pole holder on #2 Goodman Jeep at the East Finley Portal Bottom and felt pain in his left shoulder. Employee began missing on 4/15/09 due to this injury.
While laying track on 4 Section, moving rail with lo trac, operator let rail down onto employee's left foot. EMPLOYEE DID NOT START LOSING TIME UNTIL 3/19/2009.
Injured was removing old overcast when it shift causing it to hit left ankle. (2 section)
Employee was scaling out of a JLG basket. Employee scaled a large piece of rock down on to boom of the JLG, causing the boom and basket to rock down causing employee to fall out of basket. Employee struck boom with his ribs stopping with fall protection. Employee suffered cracked ribs.
3/17/09 Operator reported that he woke that morning with chest pain and believes it was contributed to the previous days work function of washing out pumps using a pry bar in an upaward pulling strain motion.
Injured was bolting top when piece of rock fell between bolt striking rt. lower leg.
Man was getting off mantrip, stepped in hole and twisted right knee.(right)
EE stated he slipped and fell, hurt his back.
Employee was checking for a leak from a drill rig hydraulic hose. As he moved hoses around, pressurized fluid shot out & caused a puncture wound to his left middle finger tip.
Employee states that he picked up an oxygen cylinder and slipped in the mud, causing him to fall down & strain the muscles in his lower back. Days missed: 3/17-3/20/09.
Injured was plastering overcast and stepped off ladder straining right ankle.
While mining in 12 I section a piece of roof stone fell from between the roof straps and struck employee on the right foot. This caused a laceration to his foot - 8 stitches to close laceration.

Employee was plastering on Int. stopping, when the ladder tipped out away from stopping, causing the employee to fall.
Employee was exposed to coal dust in his eyes. The employee had a cold in his eyes prior to working.
EE stated that while being loaded, a rock fell into the bed of his truck, causing him to be jarred in the cab.
Employee was changing a 992G Cat wheel loader tooth. When picking up the loader tooth he strained/pulled a muscle in his lower left stomach area.
EE was standing a slab of sandstone up on it's edge, when it broke in two. The bottom half of the slab struck his lower left leg and left foot, causing contusions to leg and foot.
Employee was attempying to hang a continuous miner cable. As he bent over to pick it up his back popped, causing a severe pain.
EE was installing air line on the rib when a piece of coal measuring 6"" X 6"" X 5"" fell from 12"" above his head and rolled down his arms striking him on the forehead causing a concussion/hematoma.
Employee was servicing equipment and stepped in a hole and twisted his ankle.
Employee was using a hammer and was pulling pins when he felt a cramp in his left leg.
Employee was riding rail runner in low clearance and hit his back on the top causing injury to his back and ribs.
Injury slipped and fell on 3/13/09 at 3:00 pm. He was picking up auger equipment to bring back to the garage for repairs. He fell on the flighting of the auger. He was hurt but did not go to the hospital. He was not better in 1 week. So he went for x-rays at Punxatary Hospital on 3/20/09. He has a cracked rib.
Employee was assigned failing object guard installation on mobile conveyor #1. To gain access to one of the rollers he attempted to move a 3"" power cord by himself instead of using come along and strap provided.
Cleaning and shoveling bentonite from Mill floor. Muscles in back tightened causing pain.
The employee was down and near bottom of slope, twisted his right ankle. The incident resulted in a lost time injury.
While crawling on his knees, the injured employee stated that he felt pain in both knees and lower part of both legs.
Employee was cutting #7 right at 36 crosscut when rock fell in unsupported roof area and cantalevered into supported roof area striking right ankle. Employee sustained a simple fracture of the right ankle.
While shoveling a tailpiece, the employee was struck by a piece of rib rock on the head and neck. The rock measured 30"" by 8"" and was 1"" to 4"" in thickness. The employee continued working and started missing work on 4/13/2009.
Emp was grinding metal on 3/12. emp reported on 3/13, he thought he had pink eye. he went to ER on 3/14 and they found metal in eye. he was refered to an eye doctor on 3/16 who kept him off work until 3/19 when he returned without restrictions.
The employee moved between the rib and the bucket of E-19 scoop when the operator moved the machine.
Working on roof bolter changing out bad hose & fittings. Was pulling hose towards himself to loosen a fitting & felt twinge in back. Continued to work; went to lunch around 1:00 pm & pain in his back got worse. Notified coordinator & was taken to Medcor.
Bullgang crew was using skid steer to stand up door frame for equipment doors. Frame was chained to forks. Employee was moving cable out of the way when chain came unhooked and frame fell on top of his right foot. Fracture to small bone on top of foot.
Employee was carrying 5 gal oil cans up longwall face to change oil in tailgate drive. Was crossing from rear walkway to toes of shield between 40 and 45 shields and fell hitting back and chest. Rib fractured and bruised abdomen.
EE was lifting a piece of sandstone onto a pallet, when he sustained a sharp pain to the lower back. Physician diagnosis: Low back strain/strain. Note: Monday 3/16/09 was a non-work day for the company.
Employee was re-claiming an old overcast when one of the support beams fell, striking him on his left ankle, causing a fracture.
While setting the tail piece, EE slipped and twisted his right knee. He continued to work until May 4 when he had surgery.
While unloading miner bits that were in buckets, employee felt pain in his back and numbness down his left leg. The doctor took him off work on 3/26/09. He began losing time as of that date.
Victim strained his back pulling on a 20' section of 54"" belt. He started missing work on 3/17/09
He was unclogging a chute with a 4 foot rock bar. He said he felt right shoulder pop. Took him to the ER, took an x-ray. They did not see anything wrong. He called the next morning stating his arm hurt. Came in Monday for some training in the office, did not mention his arm was sore until after training. He was released to see Dr. 3/16/09. Saw Dr. 3/23/09: 2 weeks off work.
EE was tramming shuttle car when the miner operator pulled his cable too tightly, pulling it down from hangers in front of the shuttle car. The shuttle car operator then went around his car to re-hang the cable when his right knee popped.
Drilling for roof bolt. Victim got glove caught in drill steel, twisting left arm and breaking it.
Employee was moving cribs, got left (4th) little finger caught between 2 crib blocks.
EE was carrying crib bag, tripped on rock and fell, striking right knee on rock.
EE operates a roof bolter. His hand got caught in canopy when it came down. He simply had his hand in the wrong place at the wrong time, and he knew it. It mashed his thumb so much that it broke it. Will be off a two or three weeks.
Employee was changing a return roller and a gust of wind blew dust in eye.
Employee reported: Attempting to service a belt drive, as he stepped up onto an I-beam that the drive base was set on, he experienced pain to the back of his right knee. He reported the event to his supervisor at the end of his shift. Later that day at home, his knee began to swell and he sought medical attention for a possible strain.
EE was tightening the tire chains on MG-11 and felt something pul in his right shoulder.
Employee was climbing the ladder on a Cat 777C rock truck. While climbing the ladder, he bumped his foot on the ladder which caused injury to right foot toe joint.
Person was stepping off the rear platform of the roof bolter and when doing so he stepped into a hole in the pavement twisting his left knee causing injury.
DID NOT START LOSING TIME UNTIL 9/2/09. SURGERY REQUIRED AT THIS TIME. Walking, tripped on rock, fell on knee.
The employee stated he was tramming scoop and hit a hole in the roadway which jarred his back. He took the scoop inby one crosscut and got hung up. Crib blocks were placed under the wheels and when he trammed off the blocks it jarred his back.
Injury had started the continuous miner to prepare for bading shuttle car. The shuttle car opr. signaled for her to check her cable slack on the reel. After shutting miner off, she walked to shuttle car to check the cable slack when a piece of slate rock measuring 1' wide, 2' long, and 1"" thick struck the top of hard hat causing staving but not knocking her down.
Employee was changing wear plates inside of a crusher. He was striking the plate with a hammer to loosen it. When the plate became loose, it slid down onto the employee's fingers.
While the victim was swinging his boom/canopy in the face of No. 8 entry on NO. 3 unit, a piece of material slid off the top of the roof bolter canopy and struck him on the lower back.
Miner caught the tread of his boot in a rung of an access ladder on an impact crusher while desending straining his lower back. 7000-1 not submitted. Have already been cited for this violation per Bill Coates, Green River, WY MSHA office on 06/22/2009.
Employee was stepping off a ladder of a drill when she stepped on a rock with her left foot and twisted her ankle.
Cutting open a crack on fairlead of dragline. While cutting, hot oil & grease from inside the crack, shot out striking the employee in the facial area causing burns around his burning goggles.
Employee was climbing out of a chute under #4 shaker when he felt a ""Pop"" in his right knee. Diagnosis: Right Knee Strain - possible Medial Meniscal Tear.
Roof bolting and a draw rock (1 1/2' x 2' round x 1""-3"" thick) fell striking employee on neck & lower back.
Employee picked up a box of resin and turned to the right and at such time he felt pain in his lower back. Went to the doctor on 3/12 and was taken off work. Started to lose time as of 3/13/09.
The individual was opening an air lock door, which is located on the mine travelway, and felt a pull in his lower back. He did not miss work or go to a physician until 5/19/09. The physician recently determined that the individual has a hernia. The operator was not aware that this was job related until 6/25/09.
Employee was moving a piece of rock with slate bar felt pain in lower back.
Employee was pulling a roof bolter cable over toward the mine rib and strained his right shoulder.
Employee was repositioning the continuous miner & while observing the movement of the continuous miner employee was walking backwards away from the machine, tripped & fell to the mine floor. In falling to the mine floor the employee hyper-extended his knee.
Employee was removing crib block from a foreman's pickup and co-worker shut tailgate on employee's fingers on left hand. Severe contusion of middle finger with knuckle swelling.
Roof Bolting and employee states the ATRS fell striking employee. Alleged Accident investigated by mine foreman and no sign of inoperable or malfunction ATRS found.
EE was moving the roof bolter, reached up to move an old roof bolt that was hanging down and caught his right ring finger between the roof bolter canopy and the roof bolt plate.
While backing little two man Johnson buggy up, ran over loose coal and got head in the top and stove neck up. Did not have any medical treatment til 5/5/09 and started missing work on 5/6/09.
Employee stated he was picking the miner cable up and walking it out of the roadway. After he got done he said it felt like his shoulder poped. Employee was unsure of the injury so have been waiting on the MRI to see if it wasn't an Illness.
EE pulled lower back muscles while stacking 50 pound bags onto a pallet. It was determined that she would be better off not doing this kind of work and decided to take a termination. She did not miss work until 03/11/2009.

Right side of face cut-traming scoop outside, went around turn and scooped up discharge water line that was laying along rib.
miner was setting a shield when a rock fell between the shields and struck his foot.
Employee stated while moving safety chain from counter weight structure of U-50 refuse belt he was moving loose end of chain over top of sheeting. Employee stated at this time he believes his small finger tip went thru a hole in the chain link and was struck against sheeting girt or beam at the same location.
Employee was lifting door on the roller mill fan with his right hand, slamming door onto the left hand of the employee. Left hand swelled up and took employee to the emergency room to x-ray hand & wrist. Employee received a small distal radius fracture to his left arm. Employee is estimated to be off work around 3 weeks.
While walking she tripped over belting. She continued to work but continued to have pain. She was eventually taken off work on 4/6/2009 for surgery to repair a torn meniscus to the right knee.
Employee stepped off of a piece of equipment and turned left ankle, causing a sprain to the left ankle.
Employee was removing the oil plug on top of the shearer when a piece of rock fell from between the shields and struck him on the right hand resulting in a fracture to the right little finger. Employee was wearing gloves
Employee fell to mine floor due to slick mine floor. Received an abrasion and a contusion. He worked his next scheduled shift and reported his leg as swollen and hurting. He was instructed to seek medical attention and was hospitalized with an infected leg.
Bent over to pick up wire, sprained back. Was given 5 days off to heal, managers decision.
Wet shot rock plugged up the Primary Crusher. I was inside of the crusher cleaning out the rock. I picked up a large rock to throw outside, the rock slipped off of the blow bar and fell on my left index finger, cutting it deeply.
Employee stated that he was getting out of the dozer. He stepped on a loose bolt on the deck causing him to slip. He grabbed hold of a safety rail to keep from falling. As he did, he strained his shoulder. Taken off work on 4-9-09 to have minor shoulder surgery to repair rotator cuff tear.
Employee stepped off equipment and sprained his ankle.
Employee was removing a tie down strap from a tool box and was lifting a metal tongue up and pulling on the strap. Felt pain in his lower back.
EE was removing the guard to get across to clean the chains & sprockets. While lowering guard to ground, it slipped out of his hands and landed on his right foot. Co-worker was also helping him remove the guard. This work was being performed because we were on a mini outage. Employee did not get the treatment until the 9th.
Employee was stacking crib block, he was lifting and turning without moving his feet and sprained his left knee.
Employee was using a slate bar to line a beam, when the bar slipped hitting him on the left ankle.
Employee stated he was bolting first cut in a x-cut. As he installed the drill steel into the chuck, a rock fell and struck him on the left hand, causing a laceration to his left index finger
Employee was outside scoop picking up glue for bolter when rock fell out between bolts approximately 2' x 2' x 4"" thick struck him in the lower back knocking him to ground.
Employee was lifting a D-Ring with chain after a shield had been set on the E-19 Longwall set-up face and felt a pain in his left arm above the elbow. Employee began missing work due to the injury on 3/20/09.
Employee was putting forklift on charge and stepped on uneven floor and sprained his left ankle.
Employee was cutting ventilation curtain with knife when he cut left wrist lenghth wise 3/4"" long. Employee continued to work but missed work due to this incision on 3/9/09.
The employee was eating lunch at the section transformer. A miner cable fell from the top and struck the employee twisting his back. He worked for two weeks and went to the dr on 5/19/09.
The employee was unhooking a piece of pipe connected to the bucket of an excavator when the operator engaged the emergency stop lever so he could move the bucket. The operator didn't realize his leg was depressing the bucket joystick. The bucket dropped onto the employee's left hand, pinching his fingers. Surgery was performed & ee is off work.
While splicing a conveyor belt employee felt pain in groin area as he pulled on belt.
Employee was mounting his machine when he slipped and fell to the ground injuring his left shoulder.
Employee stated that he was lifting a box of resin off of the bottom in main west section and felt pain in his back. After the results of an MRI that was conducted on March 14, 2009 and a diagnosis was determined on March 18, 2009 this accident is being reported as a lost time injury.
Employee suffered two fractures to the right foot as a result of a slip and fall incident.
Roof Bolter Operator was bending a roof bolt and felt a sharp pain in his lower back.
While shaking roll-feed gate the chain support broke a link, causing the bar he was using to slip. Injured fell on right foot.
Employee's feet got caught by the feeder cable as it was being pulled, injuring his right knee.
Employee was unloading a box out of scoop bucket when he lost his footing and fell landing on his butt.
As EE lifted one end of a 8"" plastic aqua line section of pipe and turned to push the pipe thru a mandoor, he felt a sharp pain in his lower back. EE continued to work until 3/21/2009 at which time he went to the doctor.
Cutting skirting, knife slipped, laceration to right thigh. 3 Stitches
Putting conveyor chain in miner- steel cable broke striking employee in nose.
While descending the deck ladder on the #578 Drill, employee's hat blew off and in an attempt to catch his hard hat lost grip with his right hand. This resulted in employee spraining his left wrist. He was prescribed a wrist splint for support.
While the member was carrying roof bolts from the middle of the machine toward the front tray of the bolter, he stepped in mud and tripped. The roof bolts he was carrying hit the rib keeping him from falling causing a strain to the middle portion of his back.
While cleaning conveyor belly guards under beltline a guard full of material debris dropped down, hit the manlift basket and then jammed the basket up and into the conveyor frame in a ""spring up"" manner.
The injured was in the process of changing batteries on a 488 scoop located at the Unit #2 battery station. He exited the cab of the scoop and was walking over to disconnect the jumper cable, when he stepped on a piece of roof rock measuring 4"" X 3"" X 3"", causing him to twist his left knee.
Employee stated he had gotten out of his Shuttle Car to walk away, when he stepped on uneven ground and sprained his ankle. Employee started taking off work on 03/06/2009.
EE was walking back to get a roof strap from rear of miner in #1 Bleeder Entry, E-19 Section, when a piece of rock that measured 18"" X 24"" X 3"" thick fell from between the roof straps and struck him on top of the head, jamming his neck. EE began missing work due to the injury on 3/23/09.
The injured was operating a continuos miner in Unit #1, entry #6, at 13+70, when a piece of roof rock measuring 38"" X 24"" X 3"" to 6"" thick fell from the mine roof between the last two rows of bolts striking the injured on top of his hard hat. The injured has been diagnosed with a fracture to C6 and C7 in his neck.
Was moving a bottle of Blue Wolf degreaser. It dropped and splashed in face of worker.
A mechanic was operating the shop overhead crane placing a planetary gear on a haul truck. While attempting to lift a tool to be attached to the gears the employee activited the high speed lift button, causing the tool to slip & tip the gear onto the employee's left foot.
Employee was throwing a crib tie under the belt when he smashed his right index finger between the crib tie and a bottom roller. He had a fracture and laceration to the tip of his index finger and required stitches.
Employee slipped off the last step while dismounting the front end loader. He initially told the foreman that he was OK and worked without incident for the rest of the week. On the morning of 3/8/09 he awoke to find his right leg swollen and black and blue. The ER @ Dubois Hospital diagnosed an infection under the unbroken skin on his leg. His first day off work was 03/09/2009.

EMPLOYEE WAS DESCENDING STAIRWAY IN THE MAINTENANCE SHOP AREA. EMPLOYEE DESCENDED DOWN THE STAIRWAY UNTIL HE REACHED THE LAST STEP LOSING HIS FOOTING & SLIPPING OFF OF THE LAST STEP STRIKING THE FLOOR RESULTING WITH A BRUISED LEFT KNEE & SORE BACK ON HIS LEFT SIDE. EMPLOYEE ALSO STATED THAT HIS LEFT SIDE RIBS HURT. EMPLOYEE WAS SENT TO VRMC TO BE CHECKED OUT.
Mechanic fell about 6 feet while working on customer-owned equipment. D11R. He lost his balance when he grabbed the filter bonnet which had been loosened. He fell on the tracks and landed on his right side. On the day of the incident, he finished his shift, bu awoke with his right arm stiff and swollen and his right hip and wrist sore.
Stated that he twisted his knee while kicking a rock off of a piece of belt that he was going to pull out of the mine with a scoop.
Miner was binding a load of pipe down. The chain slipped and the miner fell on his butt / back.
Employee was moving coal mill liners (approximately 60 lbs) from an elevated pallet to the coal mill. He would have to lift them up around 10 inches and place them on the lip of the coal mill for support. He was moving the liner with a twisting action and felt a pinch in his side. He did not report the accident until 3/9/2009 and comp carrier determined coverage on 3/19/09.
Employee claims he was pulling a 6 inch diameter pump hose and pulled muscles in his neck and arm.
EE and his helper were carrying a 5' x 5' screen panel on the 2nd floor. They attempted to rotate the panel. While stepping backwards, ee's foot encountered a pipe going through the floor. Helper continued to push causing ee's right knee to twist causing pain and swelling.
EE was cutting a metal band off of a bundle of roof bolts and the band flipped back and his his left eye. He had to have surgery to remove a piece of metal from his eye. Started missing work 3/5/09.
The bulldozer slid out of the road. Employee put blade down to stop. The bulldozer rolled 3 times down hill. He fractured neck in two places and had 9 staples in his head.
The employee was advancing line curtain in the face area with the use of a step ladder, when a rock fell from the roof, striking the employee and knocking him from the ladder. The employee received a strained lower back.
Employee was loading a SC in the face when a brow rolled off and hit him in the right shoulder. Due to complications he had an operation on his shoulder on May 15, 2009 and is missing work.
Employee left work due to pain in low back, saw Chiropractor. RTW 3-4-09, work until 3-11-09, return to Chiropractor; taken off work until 3-13-09. 3-13-09 saw Chiropractor, taken off work again, RTW 3-17-09.
On 3/3/09 the employee was taken to the hospital and was admitted for more than 24 hours. While the employee was descending ladder on loader, his leg gave out and he fell approximately 4 feet resulting in R-leg and pelvis fractures. (Employee has an underlying medical condition which may have been a causal factor).
Employee was trying to untangle the dredge tail line that was wrapped around the pontoons. He put a shovel under the tail line to move it and when he did, he fell into the boat on his left side and injured his left knee and left wrist.
Employee was rolling a shuttle car tire out behind a pallet of dust when he got off balance and the tire fell on his legs bending him over backwards across the end of the pallet causing him to injure his back by extending him backwards over the pallet.
EE was operating battery powered scoop. While in the process of changing batteries EE placed a crib block under scoop bucket & raised the front tires off mine floor. Scoop bucket slid off the crib block, causing scoop to jar EE. He was seen by local ER physician 3/3/09 & released to return to work. He visited his family doctor 3/9/09 & was removed from work.
The employee complained of back pain after lifting and turning and doing normal work of a scoop operator on March 3. He continued to work through April 27th without incident. He began missing work on April 28th due to back pain. He has a history of back problems. This is an occupational illness from repetitive motion trauma.
The injured was in the process of installing an 8' bolt in an intersection. As he was drilling his hole, a wing curtain got caught on the drill steel and caught his left arm, jerking it toward the steel. The injured pulled his hand back away from the steel and this caused a laceration at the base of his two middle fingers.
Employee reached to push a button while seated in a chair, resulting in pain in his back. Extent of impairment is unknown at this time.
Injury slipped on 3/3/09 coming down a ladder. He refused treatment. He worked til 4/15/09. He went off work on 4/15/09 after coming back from a 5 day suspension. He was suspended for insubordination and cursing a foreman. This accident is under protest waiting for brick street ins. I.M.E. to determine if accident is legit.
Employee walked up a 3/4"" clean gravel pile to repair a grease line on the head pulley of the stacker. When he finished he ran down the pile and fell to the ground.

Moving axle with hand dolly, employee repositioned axle on dolly and lost control of it. Axle fell over and landed on his right foot.
While installing belts on 2350PP003 employee felt a pop in his right shoulder. As the day progressed it started to get sore. Medical attention was sought. Injury was classified as lost time on 10\14\09.
Employee was shoveling rock out of trash box and turned to throw the rock and felt a sharp pain in his lower back. **EMPLOYEE DID NOT START LOSING TIME UNTIL 3/5/09**.
At approx. 10:20 AM EE went to go to the restroom. She opened office door to go out & when she stepped on the grating her foot slipped causing her to fall. Her left foot slipped out & went under the step. There was a little ice on the shaddy part of the grating. The next morning her foot was still hurting so she was sent to the Dr. & Dr. said foot was sprained & to stay off it.
The employee stated he was operating a shuttle car and ran over a rock. He got out of the shuttle car to move the rock and when he bent over he felt a pain in his back. He stated he did not touch the rock. He had been diagnosed with degerative changes in his back.

Contractor was removing Pin on tilt cylinder at the Bell Crank of a WA 900 wheel loader. When pin was removed the bell crank shifted and pinned the outside left foot below the ankle. He called other PMC mechanic to aid in the unpinning of his foot. Once he was stabilized and on the ground, EMS was called and he was transported to the nearest hospital.
Employee was pulling #27 shield over top of the shearer behind the headgate drum while standing at #26 shield when a piece of rock fell from between the shields and struck him on the calf of the left leg. He began missing work 3/5/09

Employee hurt right shoulder while pulling power cable on 3/1/09. It was not reported to Safety until he went to the clinic on 5/2/09. After an investigation was completed, it was determined to be a work related injury.

Employee was attempting to loosen a four inch drain line when the coupling he was unlatching fell and struck him in the right forearm. Employee was taken to the Hospital were he received stitches and a prescription.
The injured employee was cleaning next to a conveyor by shoveling material into a bucket and then dumping the bucket on to the belt. The belt was not running. As he dumped a bucketfull on to the belt he felt a pain in his knee. The following day, the employee visited a doctor and he was advised not to work for at least a week.
Employee was changing a bearing on a screen and while pulling hard to break a bolt loose strained his abdomin.

The EE was assigned to service the mantrip located outside the mine shop.As he was walking out of the shop, he tripped over the corner of a wooden pallet that was laying on the ground next to the shop. While he was walking out of the shop, he was carrying a quart of oil that he was going to put in the mantrip. He fell forward & landed on his right knee & right shoulder.
The original injury date was 3/28/09 when the employee was tightening an overhead bolt on the crusher and he felt a pain in his right shoulder. The employee was not placed on restricted duty or incur any lost time as a result of this injury until 5/20/09 when he had surgery.
A shovel oiler driving a Ford F350 pickup turned west to enter a haul truck staging area. He was momentarily unable to see due to the setting sun directly in his eyes and struck an employee walking from a parked haul truck to the lunch trailer. The injured employee suffered a broken ankle.
Casing was stuck on catwalk. Employee was helping to free the pipe, when it fell off catwalk and smashed his ankle.
He was shoveling material out from machines and underneath. When he went under the machine and was shoveling he felt a pop on his lower back.

EE came to work on 3/30/09 and told his supervisor he was hurt. At that time, he was told to go to the doctor. He did not report any injury to Ohil Stone, Inc on 3/26/09. His Dr report says that was the injury date.
Employee was overhead welding a cross brace in the wet screen when hot slag dripped into the gap of his long sleeve shirt where it buttons at the wrist and it gave him 2 small burns on his left elbow. The small burns developed into an infection which resulted in lost time because employee did not report the incident to his supervisor until 4/14/2009
The employee slipped on a patch of ice while walking from a loader to the control room and fell on her left side. She experienced some discomfort in her shoulder and arm. The employee was examined by an specialist on June 11th and was removed from duty for further evaluation.

Employee was walking near #3 settler, when he slipped on process scale and fell. He fell foreward on his hands. Employee complained of pain to his right knee and went to the Medical department. On April 3, we received his Doctor's diagnosis making it reportable.
Employee was sitting on the ground with his back leaning against his roof bolter waiting for the continuous miner to move. A battery powered ram car ran into the rib and mashed the bolter cable about 30 feet from bolter. The bolter operator felt a shock in his lower back.
Employee was cleaning area around a conveyor using a shovel. Employee claims he struck his wrist against the frame on the conveyor. He stated the incident occurred on 2/13/09 and sought treatment on 3/23/09. Restrictions given on 3/26/09.
Employee was climbing down off of an end loader when he slipped and fell on his left side. Employee suffered a fracture to his left arm.

Employee was cutting a hose used for the vacuum truck with an utility knife and pulled the knife towards his right hand. He suffered a laceration to his right, middle finger approximately 7mm in length. He was initially treated on site with basic first aid. This became reportable on 4/20/09 when he received surgery to repair a damaged tendon.
On 5/17/09 the employee left work with back spasms and back pain that has been attributed to the injury of 3/22/09. The employee tripped on a wheel chock rope for the pickup truck as he walked away from the truck.
The employee was hanging sample tubing. He was standing on a ladder that was leaning up against a roof support can. The ladder slipped and the employee's leg got twisted up in the ladder and he fell to the ground. Restricted duty 3-21-09.
Employee was assisting others in positioning a tilting pipe deck on the forks of a forklift. They were lifting the deck when employee felt a tear in his right shoulder.
EE was taking down monorail, pushing it up on the stageloader, when he struck his right elbow on the stageloader hanger for hoses. EE became lost time injury 5/22/09 due to surgery. Lost time reportable injury as of 5/22/09 due to surgery.
Employee installing hydraulic pump in mobile diesel equipment. Felt pain in left shoulder. After further medical evaluation on March 31, 09, employee was placed on restrictive duty.
Injured was reaching to get a bottom roller, while on move crew, was pulling back and a top roller fell, striking him in the right side of back. He finished the shift, took the next day off, returned to work on Friday then went to the dr after work on Friday.
Large rock fell on left foot just below ankle. Was wearing steel toed boots but rock hit side of foot pinching between rock and steel track. Tried calling MSHA person for questions on 3/19/09 and received answers on 3/31/09 due to phone tag and serious illness in reporters family.
Mechanic was installing a new dump cable on our dragline. While tightening the cable socket, the cable slipped out of the clamp and struck the mechanic in the face. The mechanic suffered a laceration on his upper lip and a fracture to his upper jaw.
Employee was sitting on the muck pile in the Link Belt 330LX feeding the Jaw Crusher. By accounts, the Extec Screen Plant stopped and was overflowing with material. It appears that EE tried to climb out of his equipment to turn the jaw off, slipped and fell 12 - 15 feet down the muck pile. He was found unconcious and given first aid by co-workers until help arrived.
