A roof fall was discovered at 4 mains 21x-cut #14hdg. This fall was 1,600 feet out-by the feeder. Fell above the anchorage of six foot and eight foot super twist roof bolts. The area was supplemented with 12 foot post tensioned cable bolts. Fall was in the intersection only. approx. 18' by 18' in area.

Roof fall was discovered on 02/27/09. It is in the House Extension of the mine at #44 break in the #3 entry. The fall is approximately 30Lx18'wx6'H. The fall had no affect on the ventilation or travel ways of the mines. The area has been breakered off.

Geologic conditions consisting of grey shale top mixed with stack rock. Cover of 330', area was bolted with 6' Fully Grouted Resin Bolts with hole boards and 12' non-tension cable bolts as supplement. Fall was approximately 16' by 16' and 10' in height. Notified MSHA hotline.

An unintentional roof fall occurred at the Galatia Mine New Era Portal at 5422' in the #3 Entry of the 3rd West Longwall unit, MMU 059-0. The fall was approximately 50 feet long, approximately 18-20 feet wide, and approximately 3-3 1/2 feet thick. The fall did not affect ventilation. The fall will not be cleaned up. Fall #26 D.
Discovered a roof fall in the GL4 panel 3 breaks outby spad 2593 in the #2 entry. Fall is 10' thick x 18' wide x 40' long, 4400' from the active section, developed 9/2007 and bolted with #5 x 48"" fully-grouted resin bolts. This area has been dangered off.
Supervisor was making prestart face exam and found low volume of air on face. Went to bleeder and found two brattices damaged, most likely from fall of gob. Notified CP person who began withdrawal of Miners, except those needed to repair brattice.

Hoist was down due to an electrical conductor had burnt off.
An unintentional roof fall was discovered on 4 North Mains in the #3 Entry at crosscut # 86, survey station # 18231. The fall measured 20 ft long x 20 ft wide x 10 ft high. No one was injured, ventilation was not disturbed, and no equipment was caught. This fall will not be cleaned up.
A roof fall was discovered at approximately 7:40 a.m. this date. The fall occurred in the Second East Mains, x/c 35, #4 entry. The fall measured approximately 20 feet long, 20 feet wide and 8 feet high. The fall did not impede travel of persons or affect any ventilation current.
Employee discovered a roof fall above the anchorage in the return air course. There are multiple entries and the roof fall does not effect ventilation or travel.
Rock fall occurred in 1 Left off 1 Right off 1 Left off 1 East Mains in the #5 entry starting at break #8 and extending to break #11. The rock fall was approximately 18' wide x 200' long x 9' thick.

The rock fall was approximately 5 to 6 feet high, 20 feet long and 20 feet wide.
A roof fall was discovered at north G., 2,1/2 x-cut. #0 hdg. Fell above the anchorage 18' wide 7' high, do not know how long. 6' super twist bolts and 12' post tensioned cable bolts were in use. No men or machines were injured or trapped, no ventilation or travel was affected. This fall was 2,800 feet out-by the section loading point.
A roof fall occured in a neutral entry crosscut approximately 3500' outby section near horse creek #9 belt at break #71 between #3 & #4 entries. Fall measured 50' long 18' wide & 5' thick. No injuries no passages of men affected. exact time of fall unknown, found by preshift examiner.
A roof fall was discovered during the preshift exam of 3 section, located in #4 entry in the intersection inby spad 25336. The estimated size of the roof fall is 20' wide X 20' wide X 8' high. No injuries were reported as a result of the roof fall.
Ice accumulations on track and door switch. Removed ice and adjusted door switch.
Operator drove the excavator into a sump hole. He was in an area that he should not have been. The operator was trapped inside the cab for approxomately 1.5 hours. The operator was extracted and had no injuries.
Condensed water in the air line going to the left brake system on hoist 4701 froze. This made 4701 hoist inoperable until the air lines going to the left brake system were un-thawed.
While trying to start the #3 coal mill in order to empty it the pressure relief vents opened. An ignition of coal dust occurred in the coal mill system ductwork. Ignition source was in the system recirculation duct.
Roof fall occurred on the #9 converyor belt. The roof fall was found at approx 10:00 AM. The roof fall is located 2 cross cuts outby the #9 belt head drive. Roof fall is in the #4 entry approx 65' inby survey station 4135. The roof fall is approx 60' long X 20' wide X 8'-12' thick. This fall is outby the active section. Roof fall reported to MSHA on 2/21/09 @ 10:25 AM.
The Longwall experienced the first fall of roof in the gob behind the shields. The fall damaged a few ventilation devices behind the set up entry. A brattice, a set of run through doors, a regulator, and 9 curtains were damaged.
Rock fall occurred in the 2 East Mains at break #6 in the 4 right crosscut and extended to and included the #5 heading. Fall measured approximately 18' wide x 55' long x 8' high.
HAD A ROOF FALL IN PRIMARY ESCAPEWAY, SET JACKS AROUND FALL. WE RE-ROUTED THE AIRWAY AND PUT AIR LOCK DOORS UP.
A roof fall was discovered at 3 main 26 to 27x-cut #5 hdg., and into the #27x-cut from #5 to #4. Fell above the anchorage of the six foot fully grouted roof bolts, cannot tell how high. Fell rib to rib for 90 feet in the #5 hdg., and rib to rib for 80 feet in the #5 to #4 x-cut.
A non-injury roof fall was discovered in the intersection in front of #4 seal off of #8 entry in the Northwest mains, spad 589. Fall was approx. 20' x 20' x 6'. Fall did not block access to the seal. Additional support in the form of crib installed. Ventilation not affected by fall but rerouted to allow safe travel of airway.
The hoist went down at 8:30 AM called MSHA 1-800-746-1553 at 9:25 AM to report it. the problem was the AC breaker not staying closed due to faulty output from current sensor unit. Reset the unit and rebooted all PLC's the initial fault caused by brake position micro - switch out of adjustment
A roof fall was discovered at 4 Main #9x-cut 2x1 and #2 hdg at #9x-cut. Fell above the anchorage of the six foot super twist bolts, cannot tell how high. Fell rib to rib for 20' in the x-cut and 20' in the hdg. Area was already timbered off with wood cribs and wood props.
Crew realized that water was coming into the mine in two different areas.
A proximity switch on one of the gates was repaired and a test trip made. When the conveyance was brought to the upper track limit a control breaker tripped. Efforts to diagnose the problem failed to bring a solution in a timely fashion. MSHA was notified and an evacuation of the mine was undertaken. All employees were brought to surface.

Brake car jumped off track while lowering FCT underground. Down from 1:02 pm to 3:52 pm.
Possible alleged ignition on the 003 MMU in the #5 Face, 130' inby spad 7190. Miner man thought he seen a flash, while cutting coal for 2nd car. Foreman who was also there reported that he didn't see anything. They shut the miner down and then continued to examine the miner and area, and found no sign of ignition.
Unintentional roof fall, #78 crosscut #4 entry unit #3 North Submain. Length 25' width 18', height 8'. Fall propped all 4 sides, 4 way intersection. Date of development 1-5-09.
Grass fire started off of mine property near the 51-A highway and blew onto mine property. Extreme dry weather and windy conditions contributed to the fire. Canton and Fairview fire departments responded to our calls and extinguished the blaze. There was no damage. Employee called MSHA hotline for immediately reportable incidents at 3:15 pm on 2/16/09.
Knight Hawk Coal LLC, Royal Falcon Mine has had an unplanned roof fall in the 1st E. Panel at the +580' mark in the #5 entry. Plans have been sent to MSHA to clean up the roof fall. No one was injured or hurt in the fall. No equipment was involved in the fall.
Roof fall from #41 crosscut to #45 crosscut in A-1 butt.
During a preshift inspection the fall was discovered in #4 right entry on Laurel Mains section.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to a failed diode assembly and blown fuses. The diode assembly and fuses were replaced and the elevator placed back into service @ 4 PM. No individuals were on the elevator at the time. The mine was idle at the time of the outage.
Roof fall 3-Rt. Submain B-4, at face from initial 3-4 pushover thru intersection of # 3 entry into 2-3 pushover, stopping before intersection of #2 entry. Fall 18'W X 90'L X 7'H. Sandstone channel and associated clay veins come down into roof strata. No one injured.
Was determined by Schindler Elevator Company that the tail cable on the Sugar Run elevator needed to be replaced. The elevator stopped running near the surface landing.
During the weekly inspection a tear in the shaft lining interfered with lowering the escape capsule.
An unintentional roof fall occurred in x-cut. 35' long x 19' wide x 6' thick. Wet slicky sided slip formation, bolted with 6' tension rebar bolts, to the right of spad # 168 between # 7 and # 8 entry, 1st north west mains (intake).
# 3 production hoist tripped on a drive converter A fault. Electrician cleaned the auxiliary contacts on the drive converter and then reset the hoist. Hoist was back in service at 10:05am. There were still two operable escapeways/hoists at the time of the incident.
A roof fall occured in the #6 entry about 1500 feet off the # 3 section, 1 break inby spad # 43314. The fall measured to be about 19 foot wide, 20 foot long and 6 to 8 foot high. The area was bolted with 5 foot torque tension bolts. The fall will not be cleaned up, it will be dangered and cribbed off.
A fire was reported at 9:02 a.m. on the coal rejects pile. The loader operator confirmed that the coal was smoldering on the NE corner of the pile. The loader operator dug out the smoldering coal, spread it on a flat area, sprayed water on it and compacted the material with the loader. Fire was out by 11:30 a.m.
Because of storm damage the day prior, employee was cutting metal supports on door of building when it caught fire damaging the building and several pieces of mining equipment.
A non-injury unintentional roof fall occurrred at the above mine in #6 entry, intake in an intersection at spad #275, X-cut #50 adjacent to #1 main line belt line. The fall is approximately 30' long, 25' wide and 10' to 12' thick. Roof is shale which gave way. 4' fully grouted rods was initial support.

An electrical problem occurred on the emergency hoist causing it to be out of service at 1:30 PM continuing for more than 30 minutes.
Mine power knocked and elevator primary and secondary switches were tripped.
A wind storm occurred in the area and electrical power to the mine, including the main hoist, was lost at 12:50 PM and remained off for several hours. Therefore, the main hoist was inoperable for more than 30 minutes.
A roof fall was discovered by the fireboss and MSHA inspector while making the return entries. The fall measured 120 ft in length x 19 ft in width x 10 ft high. It is appx. 7,900 ft from the section in the #1 entry at spad # 3108. The fall has been breakered off and will be rehabilitated. The area was bolted with 6 ft torque tension bolts. No rider seam is present.
An unplanned roof fall was discovered by fireboss & MSHA inspector while traveling the return. The fall measured 100 ft in length x 19 ft in width x 7 ft high. 15,000 ft from the section. 3 x-cuts inby spad 2904. All entries are breakered off. The area will not be rehabilitated. It is in an old panel.
A roof fall occurred at spad #562. The fall measured approximately 25' long x 20' wide x 5' high. The fall will not be cleaned up. The fall has been timbered off. The fall did affect passage of men or ventilation.
Had roof fall in #2 rm, 3 brks outby spad #320 in return airway. Top had several surface cracks and water was coming from cracks. Top had lamination of sandstone and slate.

A roof fall was found at 7:05 am on 2/9/09 at survey station 12+10 room 2 6rms. Fall was in a four way intersection that had been mined january 2009 and was bolted with six foot full resin pins. a signifigant clay dyke was present in intersection and addition props had been installed in adjacent rooms. Roof was supported with 48 in resin bolts. Mixed stratam no coal rider seam present. Fall was breakered off, not rehabilitated. WVOMSA inspector discovered fall while checking belts.

An unplanned roof fall has occurred in the No. 1b conveyor belt entry at survey spad No.246, the fall measures 30' in length, 20' wide, and 3/4' thick, the fall does not impede travel or restrict ventilation in this area.
Fall at 216 block on the # 4 beltline. Fall was above the 5' resin bolts. 962' of overburden in this area. This area was mined in November of 1995.
A roof fall was found in the #3 track entry, the fall extended into #1 and #2 entry. The fall was approx. 6'x140'x60' the affected area in 3 entry has been cleaned up, cribs and arches were built, no equipment involved or ventilation effected.
Outby roof fall Crosscut 11 of #4 Entry, 1-Rt Sub-main approx. 15' X 20' X 10' in height, no one injured, no equiptment involved Fall partially blocked primary escapeway and ventilation was not significantly affected.
Roof Fall in the #7 Entry of the #4 Panel off of Blue Tick Mains at spad # 1632. The Fall is 4000' from an active Section . No airways or passages is blocked and no injuries. Roof Fall is 40'x20'x5'.
At ~ 10:15pm 2/7/09 a natural gas explosion/fire severely damaged the lab and office bldg. Natural gas leaked from either a heater or gas supply line to the heater. No injuries or fatalities resulted from the incident. The adjacent cemnet plant heard the explosion, called the fire Dept. and made the first MSHA notice. A second MSHA notice was made by Systech to (800) 746-1553.
The failure of the hoist hydraulic brake system components that required alternative means of agress is an incident that is immediately reportable to MSHA as required by Part 50. The hoist was down for six days.
Operator on #34226 Cat loader was loading out 3 haulers in the DII area pit just west of the #4 ramp. As he was loading #32039 hauler, a spoil slide occurred (approx 120' long by 45' wide). The slide pushed the rear of the hauler over approx 3' into loose coal causing the hauler to become stuck. The loader & haul operator along with two others were able to walk to a safe location.
A roof fall occurred in the #9 seam in the 2 Main East in #3 entry in the pillarline between crosscut #35 and crosscut #36 measuring appoximately 35ft. x 19ft. x 5ft. to 6ft. high. This area was supported with 5 ft. fully grouted roof bolts.
IGNITION The longwall shearer was cutting coal and rock from the tailgate to the headgate when an ignition occurred on the tailgate drum at No. 79 Shield. The ignition lasted approximately 2 to 3 seconds and the water sprays put the ignition out.
A contractor, Buchannon Pump, was in the process of de-capping the 205' ft. depth de-watering shaft of 2' dia. The contractor using a torch to remove tac weld ignited methane gas inside the 2' metal casing. There were no injuries or property damages.
A roof fall occurred in the #9 seam in the 2 Main East at crosscut #57 in the pillarline between #1 and #2 entries measuring approximately 25ft. 18ft. x 12ft. high. This area was supported with 5ft. fully grouted roof bolts.
Discovered roof fall on weekly run in the 1st sw butt at spad #10 in #3 entry fall was from #3 entry to #2 entry through vent 3 to 2 fall was 19' wide 75' long and 6' to 8' high entry had be bolted with 4' fully grouted rods. There was also roof water this was in a low cover area 150' of cover area was dangered off with cribs.
Operator pulled his drill to the edge of drill bench to start drilling. When he put his mast up and turned the carousel to extract a drill stem, the stem became lodged. The operator pulled the drill up to allow room for the mechanic to work on it and the drill slipped off edge and fell on its side. No one Hurt !!!
The pinion brake on the # 2 hoist drive would not release. The mine evacuation was complete at 1:50pm. A new pinion brake releif valve was installed, the hoist was tested and restored to service. Miners returned underground at 3:00 pm.
Unplanned roof fell ni 6th Southeast Butt Room #24. 20' outby spad 3960. The fall was 30' long X 20' wide X 6' high. The roof was known bad roof had 48' cable bolts in intersections and was bolted with 6' fully grouted resin bolts. There was also 30""X30"" cribs set inby this fall.
An unintentional roof fall occurred in the tailgate of the H longwall. The roof fall occurred at 44 xcut @ss 116+77. The fall measured approx. 10 ft long by 15.5 ft wide by 8 ft high.
A rock fall occurred on 1 Left Mains off 2 West at break #37. Fall measured approximately 20' wide x 50' long x 5'-6' thick.
An ignition of gas or dust was determined to have occurred at the face of the number 2 entry of the 5 right working section.
5 North shaft escape hoist would not operate properly. Production ceased in that area of the mine until the hoist capsule truck was installed on the surface & operational. Mining resumed at 12:12 PM.
A belt fire was identified approximately 12 hours after contractors had conducted hotwork activities in the area. It is believed that accumulated combustibles between the head drum and belt smoldered for a period of time prior to the fire starting.
Area was bolted with 6' & 42"" fully grouted bolts. Area had numerous clay veins. Roof fall measures approx 50' long x 18' wide x 5-8' high.
This is a mine fire which started along Lee County, Texas road 309, and burned onto mine property prior to being discovered. This is an injury free event; however, the fire burned out of control more that 30-minutes. The fire consumed approximately 25 acres, and was extinguished by the local Fire Departments by 3:30 PM on 02/02/2009.
WANA supply elevator would not operate due to a bad emergency switch which was replaced. The elevator was back in operation 8:20am 2-2-09. No person was on the elevator at this time.
Coal was being mined in the #3 entry of 3 South MMU-002. 10 foot cut was taken on the left side and miner moved over and cut 15 feet on the right side when low oxygen & high heat was encountered. A void was located in front of the miner app. 130 feet inby x-cut 24 (The area was sealed off on 2/5/09)
Intersection #2 Entry 17+02 outby 1 X cut roof fell above the bolts which were 6.5 T-2 Channel 8' cable. The foreman found the fall approximately 7:45 AM.
Roof fall in #3 entry, one crosscut inby spad #995 on old 1st right panel. Fall was 2900' from the active section. No injuries or airways blocked. Fall was 80'x 18'x 4'.
EE was driving up to mine site doing pre-shift, when fire occurred under frame. Truck was engulfed and a total loss. Unit just recently had had major maintenance. Truck had 190,000 miles on it.
A rock fall was found at crosscut #45 blocking the primary escapeway (#7 entry) by the pre-shift foreman. The fall is approximately 35' long X 5' high X 20' wide.
Unintentional fall of mine roof located in the #7 entry at 27 cross cut on the North West Mains panel, spad # 0822. The fall is approx 35' long X 18' wide X 6' high and is 650' from the working section.
The elevator at Cook's Branch became inoperative for more than 30 minutes while the daily examination was being conducted. EE was performing the exam when the elevator stopped. The EE was released at 10:45 am from the elevator and repairs were made to restore the elevator to working condition.
Installing a rolling steel door, the victim was on a stepladder - Lost his balance and fell approximately 7 feet landing on a concrete floor. He died from his injuries. He was taken by helicopter to the hospital (St. Mary's in Evansville, IN.)
Employee was walking at the plant facilities, a cornerstone of the crane collapsed, hitting the employee directly to the body and falling down. Employee was wearing safety equipment as required (capacete). Received multiple injuries to the upper and lower body.
A truck driver was delivering a load of lumber to the mine site. While the fork truck operator was unloading the truck, the truck driver walked around the other side of the truck when the fork truck operator accidentally pushed over a bundle of lumber, crushing the truck driver fatally.
Truck out of control, driver not wearing seatbelt, ejected & crushed.
Injured was installing a yellow rope cable hanger. Injured jumped up & hooked the hanger hook over the roof bolt plate & when he came down he caught the last 3 fingers of his right hand in a loop of rope tearing skin off end of pinky finger & bruising the other two fingers. The man was left hanging by his fingers & coworker had to lift him up to get fingers loose.
Employee was helping raise a 30' long beam up a hoist opening by use of hoist. While manuvering the beam through the 2nd floor, his right hand was caught by the pinch point between the steel beam and adjacent 12"" metal pipe.
While back flushing the emulsion system on the 14A stage loader, the employee was struck in the face by a hose.
Employee was spooling his cable back on his trucks boom, when he used his left hand to guide the cable. He got his left index finger caught with the cable, causing a skin amputation to the tip of his left index finger.
Pulling grease hose off reel through door at rear of service truck, handle got caught on door, depressed lever and grease was injected into left hand between middle and ring finger.
Employee removed the pin on the Index stacker and a rock fell in the hole. He stuck his finger in the hole and the wheel was turned and pinched the employee's finger.
Employee was assembling a new 993 wheel loader. Employee was removing bolts that secured rollover protective structure to shipping pallet. When employee cut the first bolt the ROPS shifted breaking the other bolts and fell on employee's foot.
Lifting cables on impactor - lower strain.
Employee was finishing up the 6 right cross-cut. He was standing apprx. 6' from the 5 right cross-cut when a piece of drawrock fell knocking him backwards and then striking him above the left ankle causing compound fractures.
The employee stated that he was sitting in the deck of a locomotive when the safety chain hook caught his finger between the frame of the locomotive and the hook and avulsed his little finger.
Replacing rt side bushing of a transmission on a Komatsu WA500 loader. Transmission fell 1 1/2 inches and smashed rt thumb in between the mounting brackets. The thumb was cut off @ 1st knuckle.
The employee was bolting on the right side, the bolt was at an angle and he was using a wrench to hold it. The bolt spun and caused his finger to be smashed between the jaws and wrench.
Employee was sliding 3/4 inch by 4'x 4' piece of metal off a scoop, when the metal hit the ground it rocked back, smashing his little finger on left hand between scoop and piece of metal.
The employee stated he was in a mantrip coming outside. The operator ran into the back of another mantrip that had stopped in front of their trip to let a motor come out of the side track. The employee has been diagnosed with degenerative disease in the back.
On 2/28/09 the EE was getting off a mantrip and twisted his knee. It has since started causing him pain and had minor surgery on 5/14/09.
He was reaching for a drill steel when a piece of rock fell and struck his left hand causing a contusion to the left hand.
Employee was drilling a hole with the roof bolter, when a rock fell and struck his left upper thigh, causing a contusion.
Employee tripped entering dozer and caught himself by grabbing handrail to keep from falling. He twisted his left arm.
He was walking across driveway toward the Lab Building after putting trash in dumpster. His right ankle rolled out from under him causing him to fall. He fractured and partially dislocated his right ankle. The driveway is level and covered with 1B stone.
Employee stated that he was helping move the miner cable when he felt a strain in his right shoulder. Employee's FLWD was not until 4/29/09 due to surgery.
Employee was carrying wooden header blocks to a roof bolting machine and felt pain in his groin. The employee did not begin to lose time until 3/2/09.
Employee has had discomfort for a while in the lower abdomen. The dicomfort has progressed and with his normal activities on this day and the progression of discomfort he wanted to turn in a report. Employee has worked until 7/7/2009 when he entered the hospital for inguinal hernia surgery. This type is related to birth defects, other surgeries or wear and tear.
EE was loading his tools in the bed of a pick up truck. As he was exiting the bed, he slipped and fell of the truck, landing on a fork of a fork lift that was sitting beside the truck.
The employee was struck in the head and shoulder with a belt roller suspended with chain and roller bracket. The bracket bolt came out causing the roller to fall out under pressure from pulling 48"" belt with shieve rope hooked to scoop.
Employee was struck by a drag rail that a scoop was pulling as it swung around a corner. Cause of the injury was safety awareness. Compliance with rule/regs not a factor, Mining equip./systems not a factor, Miner proficiency a factor, protective equipment not a factor.
Employee place 6x6 timber under stab jack of continous miner and inadvertently placed right hand in between timber and rear, bumper. As machine jack was moved hand was pinched. He sustained simple fracture of middle finger and carpal bone in hand.
The employee stated that while walking down the beltline he stepped in a hole and twisted his ankle. The hole was approximately 14"" wide, 17"" deep and 2' long.
Pulling on drill to unchuck 6th steel, felt pain in stomach, hernia.
EE was carrying belt rollers through the crosscut when he tripped on metal band canding falling onto left shoulder. EE continued to work until 09/10/09 when he had surgery on shoulder.
Wash pushing rock, when blade caught, jarring employee. EE immediatley complained of back pain in lower back.
The man was moving a water pump when he felt a pain in his right shoulder.
Employee was delivering a load of limestone @ the plant for resale. While dumping load, the truck and trailer turned over. Employee hit his left leg on steering wheel. Inner thigh swelled up.
Employee was operating continuous miner when the cutter head caught a loose piece of metal in the mine floor. The piece of metal flew back and struck his left leg. The employee received 20 stitches in his leg.
Employee was going throw airlock door when the door. sliped and hit his lower left leg bruise it. He did not go off work until 3/6/29
Employee had just finished fueling his locomotive and in the process of dismounting the loco to replace the nozzle, he stepped on the fuel hose and rolled his ankle. This resulted in the employee spraining his ankle.
EE was standing at the back of the miner on the miner cable with the right hand resting on the roof straps. The operator swung the boom catching the right pinky and ring fingers. He received sutures to the pinky and fracture to the 1st digit of the ring finger.
Employee was operating a shuttle car when he hit a hole resulting in a bruised hip.
EE felt pain in his back after falling while carrying 3"" lay-flat hose.
The employee had just finished adjusting the ventilation curtain preparation for minng. He crawled outby to the right inby corner of the intersection and was on his knees talking with another employee. A piece of roof rock measuring approx. 60""x60""x1-1/2""to 3"" fell at angle from between two rows of bolts. The rock struck the employee on the back and shoulders.
EE failed to follow training and company required procedures. EE did not shut down, lock out/tag out equipment before removing a root from a moving converyor belt. Injury to top of left hand. Damage/Impairment unknown at this time.
EE was walking to the shuttle car snub in #2 Entry, F-19 Section, when he stepped in a shollow dip and twisted his right knee.
Employee was operating an ATV, when he hit a rock. The employee went forward over the handle bars and landed on his shoulder.
Employee was using a cutting torch to cut a piece of steel(on his back beneath the work) and a piece of hot metal entered his ear canal.
The injured employee was putting roof bolts into the roof, water was dripping from the roof and got the injured wet. While holding onto the drill steel, he felt an electrical shock in his arms and chest.
Employee backed his loader into a rock truck who was positioned to be loaded resulting in a strain to his back.
Employee was on side of hill marking the permit boundry. Some snow was still onthe ground and employee slipped and rolled his left ankle. Employee finished (reported injury at end of the day). Going to doctor 2/26/09
Employee was assisting another employee lift a part from a platform to a loading dock. Employee's foot became entangled in the overhead hoist chain, which resulted in employee tripping and falling backwards off the platform.
Injured was removing cable from a shuttlecar cable snub. He caught his left index finger in the shock absorber cylinder of the anchor fracturing it. Injured was off work from 2/26/09 until 3/2/09. He returned to his normal job at that time.
Welding cutter head hyd/jack 14-15 joy cont/miner. Punctured hole in hyd/jack. Oil spray on both hands and face causing 2nd and 3rd degree burns.
While pulling longwall supports at shear a rock fell off shear body and struck on top of left foot fracturing big toe. Person was wearing metatarsal boots at the time of the accident.
Employee was pushing rock on drill bench, rock became caught under blade raising dozer off ground and dropping suddenly, operator felt pain in lower back.
EE was de-boarding ladder on the 88 loader when hip stiffened up (was actually back pain) and became difficult to move, essentially immobile. After talking with the EE had experienced back pain from activities around the home over the weekend and the morning prior to coming to work. EE back pain substantially worsend at work while walking down equipment de-boarding ladder.
Employee was positioning his rock truck to be loaded when the loader backed into the side of the truck causing him to hit his head on the side door.
The employee was leaning over the RBM to view the reel chain to the cable reel when a bundle of roof bolt plates fell off of canopy (Tram Deck) striking his right hand.

Fracture in the bottom of bed mast causing loss of hydraulic fluid and rapid bed drop jarring truck driver seated in truck cab. Lower back injuries - muscle strain and L1 vertebrae fracture.
Employee was reeling cable up on MRS when a cable broke loose from breakaway hangers striking employee in the neck.
Emp was moving a loading shovel from one ramp to another. The shovel spun out on the slope causing the cable to tighten. He cut the rope that held the cable that was tied off to the shovel. When he cut the rope the cable whipped, knocking him to the ground. When he fell to the ground he landed on his right shoulder.
Employee was leaving roof bolter to go get on mantrip. He walked through the #2 intersection when he rolled his ankle, twisting it. The employee worked 1/2 of his shift on 2/25/09.
Employee was removing stuck drill from drill hole pulled stomach or groin muscle.
Employee was unloading concrete block when he kicked a block from between the skid and the scoop bucket. The block was holding the skid and the bucket apart. When the block came out the employee's foot got pinched between the bucket and the skid of blocks.
EE was lowering a ram car battery lid in order to use it. He lifted the lid with his left hand & used his right hand to push the prop bar down. His left hand slipped on the lid & the lid lowered onto his right hand catching it between the lid and battery case. There was a laceration and fracture to the 4th finger of his right hand between the first joint & his finger tip.
Employee twisted right knee while working on a truck.
Stepping down from drop deck of semi-trailer, lost footing, fell and broke right knee and leg.
After putting antifreeze in the coal truck, he slipped while dismounting and fell approx 3' to the ground.
While attempting to set latch pins on the Highwall Miner, EE pulled a muscle in his back while swinging a sledge hammer. He then fell backwards, causing a compression fracture to a vertebra in the middle part of his back.
On 2/23/09, EE was in the process of dismounting the D10R Dozer he was operating, when his foot slipped off the step, causing him to put strain on his right arm and shoulder. EE continued to work and did not seek medical treatment until 3/3/09 and again on 4/13/09. The first day EE missed was for pre-surgery testing on 7/27/09.
The operator was tramming the shuttle car and hit a hole in the haul road and hit his head on the shuttle car canopy. The operator sustained a bulged disc in his neck area.
Injured was walking toward the side door entrance of the warehouse when he slipped on ice and snow. Falling on his left hip and striking his cap lamp battery against the pavement. He was transported to the hospital where he was treated for contusions and pain of the left hip.
Victim was pulling the miner cable ( cutting slack) when he lost his footing and slipped and fell. He hit on his left side hip or back causing a bruise or hurting of some type.
Employee mounted and dismounted multiple pieces of equipment, around noon his ankle started hurting. He was taken to medical facilities and had a sprained ankle. The individual does not know of a specific incident that contributed to the sprain.
Employee was unhooking a cable from the back of a diesel ride when the ride rolled back and the bumper caught his foot.
Employee was on top of vehicle loading it, when driver pulled out from under bin. Employee went to hospital, was released with no restrictions. Employee still complained of pain, went to doctor and was told ""No work until you revisit doctor"", resulting in lost time.
Employee was clearing a jammed rock from the jaws of the rock crusher with a long metal tool. The jaw caught the tool and thrust it upward and hit him in the throat/neck area.
Employee slipped on the first step of a stair case, due to an oil slip on the step.
EE was pushing a bolt in top, when he felt a pop in his shoulder. EE has continued to work until his scheduled surgery date of 6/11/09. Reportable lost time accident 6/11/09.
Employee was shoveling material onto a conveyor belt in the raw coal tunnel when he experienced pain in his lower back. He did not report this accident until February 27th, 2009.
He was carrying crib blocks and placing them in the bucket of a scoop and felt pain in his right shoulder. He continued to work at his regular duties until February 26, 2009 when he was removed from work for a Rhomboid tear of the right shoulder.
Employee was working off of a ladder and his boots were muddy causing him to lose traction and slip. He fell off of the ladder and landed on his elbow resulting in two fractured bones.
Employee was hooking up light plant to his pick up, he was liptims upon the ongue of light plant to align it with the hitch on his pickup. The light plant rolled forward and caught his middle finger on his left hand between the light plant and the trailer hitch.
The employee was lifting a set of batteries out of a scoop when he felt pain in his right shoulder. He continued to work until he had surgery on 6/11/09. 6/11/09 is when he started missing time.

EE was turning to place a bag on the conveyor, when he felt a pull in his front lower right abdomenal area.
He was performing a fire run when he was exiting the clean coal silo he slipped on ice and felt pain in his left knee. He coninued to work at his regular duties but continued have pain. He was removed from work on March 18 2009 for surgery for a torn miniscus. The MRI indicates degenerative changes and patellofemoral joint arthritis. This is Occupation illness MRI result
Employee was found laying on the ground unconcious. Ambulance was called and employee was taken to hospital.
The EE was starting to work when he decided to change the battery of the 155 machinery. He jumped from the chain of the machinery with the battery in his hand and he hurt his back when he fell. He worked all day without reporting this to his supervisor. On Monday, he went to CFE and then they notified his employer.
Employee had almost completed loading a shuttle car. A rock measuring 91"" x 13"" x 11.5"" fell from top and landed on the boom of the miner. It traveled the conveyor to the shuttle car. Since the car was almost loaded the rock slid off the side of the shuttle car and struck employee in the left leg.
Employee was initially injured on 6/19/2008 (E-Document #638715). Employee returned back to work on 1/28/2009. Went on his 7 days off on 02/12/2009 and then went to his personal doctor on 02/20/2009. A new 7000-1 is being submitted. We were not notified until 02/24/2009.
Employee pulled a broken measuring tape from the muck pile and cut his finger.
Coal truck driver was traveling in 4th gear at approximately 10-12 mph when he heard a noise. After hearing the noise, he let off the fuel, causing the load to shift & the truck to overturn.
While waiting on other pinner man to finish putting up bolts, a rock came down on his arm and wrist.
Employee was opening door, slipped and lost balance and fell on bit buckets, causing injury to ribs.
Employee was working in Section ""J"" cell, line 30, when a blank cathode got hung up on a sleeper. The blank cathode came off of the strongback, landing on the employee's left foot.
Employee stated that he was picking up a bucket of plaster with both hands when he felt a pain in both arms. Employee's FLWD did not occur until 2/23/09.
While washing a Roof Bolting Machine he stepped in a hole and twisted his left knee. He did not begin missing until April 2, 2009 after a surgical procedure was preformed on his left knee.
EE was bending a roof bolt, when he felt pain in his left shoulder. EE began loosing time as of 2/23/09.
Employee was getting out of foreman's truck when he tripped on a flashlight wire and fell on his left side on ground straining his left leg.
A 992 Endloader bucket was being lowered to dump load in a 777 rock truck. During this action the boom began to free fall striking rock truck.
Employee was crossing a run-through tailpiece. As he stepped down he stepped on a turnbuckle which caused him to fall. His boots were muddy.
When attempting to hand a continuous miner cable, experienced pain to his right knee. He alleges he stepped in a hole when hanging the cable. Continues to experience pain and swelling to the knee. Had MRI done on 2/26/09, determined he had a torn meniscus. Started missing schedule work day on 2/27/09.
The employee stated he was standing at the inby corner of break 6 to 7, 100' inby spad 12591. The continuous miner was finishing a lift and the employee was getting ready to operate the MRS. He stated that a piece of coal fell from the rib and struck him on the right side of his head.
Employee said he was picking up broken 50lb bags of talc from the floor and dumping them into carts to be disposed of. While shaking out a bag in the cart, he he said he felt pain in his lower back. Employee was seen by Doctor on 2-23-09. Employee started losing time 2-24-09.
Employee stated that while dragging 2 joints of rockdust hose with fellow employee. Person along the 4 right belt he felt a sharp pain in his right knee.
Forklift was moving full pallets. Driver did not see injury walk in front of him. He did not see forklift. He was knocked down and sprain ankle.
Employee was cleaning up stuff around yard, he was moving some wooden blocks. When he picked up the wooden blocks he felt a pain in left region of lower back.
Employee was walking over to machine buggy to get parts, and said he twisted his right knee causing pain and swelling.
Worker caught finger in door, no special conditions contributing to accident.
Employee stated that while striking a conveyor chain with a hammer, a piece of metal chipped off the chain and went underneath the bottom of his safety glass, striking him in the eye.
No broken bones - Took him to Clarksburg Hospital. Put 2 stitches in and released. Left hand above wrist - no broken bones.
2 employees were attempting to start a 440 Case skid loader by putting a battery charger on the battery. When injured hooked the battery charger to the battery, co-worker started charger. The battery on skid loader sparked & injured turned to look at it. As soon a injured turned to look, the battery exploded in his face.
The man tripped on the steps in the lamp house. While falling, the man struck his hand on the check-in board and lacerated his hand.
Employee was setting timbers when he hurt his back.
Changing the track pads on the dozer and smashed right thumb with a track pad.
Walking, stumbled and fell hurting right arm. Surgery required, Torn bicep.
Employee was lifting a piece of belt structure when he felt a pain in his stomach. He continued to work until 3/25/09 at which time he had surgery to repair a hernia in his stomach. He started losing time after his surgery.
Employee slipped on a flat rock covered in mud and fell. Injured right hip and back when he fell. Started to be Lost Time on 2/27/09 per Doctor's orders.
EE felt a pull in his left shoulder while climbing a ladder of a rock truck. EE did not miss any work due to this accident until 3/2/09.
He was walking along the stage loader slipped in the mud and twisted his left knee. He continued to work at his regular duties but continued to have pain. He was removed from work for surgery to repair a torn miniscus of the left knee on 6/11/09.
miner was roof bolting and his drill steel became stopped up. a co-worker was striking the steel attempting to unstop it missed and struck the miner in the face.
Employee was walking to his Dozer when he stepped into a drill hole breaking his right leg.
EE stated that while he was installing the inside bolt in #4 entry of 5 Rt future longwall setup face his drill steel plugged up & he dropped the drill head off to check the chuck section of bolter. As he dropped the drill head off the steel stuck in the hole. He placed his rt hand on drill head to check the suction & a section of drill steel fell & struck his rt hand.
EE landed on right elbow from tripping on loader cable. Began missing work on 10/29/09, will have surgery on 11/2/09.
EE was climbing down out of caterpillar D9-N bulldozer and lost his footing and fell to the ground.
Employee was connecting the scoop battery charging cable into the recepticle on the scoop's batteries, when arcing occurred.
Injured was attempting to open sliding doors when they appeared to be stuck. He pulled harder and hand slipped causing it to hit frame.
Employee was pulling bolts off roof bolter when he felt pain in his left rib.
Racking belt on 1st North West Mains #5 entry. On top of belt pulling back onto pile. Stumbled, fell, struck right shoulder on scoop ram.
Employee was walking in an entry to set a pump and slipped and caught his fall.
Supervisor was initiating the start-up process of the Roller Mill,by overload of material in the mill table,he proceeded to rotate the table with the auxiliary motor. The auxiliary motor has a manual coupling to start-up the engine in local; the handle had a pneumatic coupling mechanism; it failed to connect properly causing the handle to bounce back hitting his right hand.
Employee stepped on a rock and twisted his left knee when carrying comealongs. **EMPLOYEE DID NOT START LOSING TIME UNTIL 3/12/09 DUE TO SURGERY**(Misc Mail 2/16/2010 #14-started losing time again on 2/10/10 due to another surgery)
Employee in TC Transport truck, states he was on 7-Pines Surface and in dodging a rock truck, hit a pot hole and seat bottomed out.
While tramming a scoop in the #3 entry EE hit a hole in the bottom causing his head to hit the canopy. Due to experiencing pain in his neck he went to the hospital and was referred to a specialist for an MRI. On 03-05-2009 EE informed the Safety Department that he had been diagnosed with a bulging disc in his neck and not to work and attend therapy.
Small ledge in roadway, s/c ran over it causing the s/c to bounce man into canopy striking his head.
EE was preparing to weld new steps. He was standing in door opening and air caught door and swung closed. Door hit right elbow/forearm and knocked him down on left wrist. After continuous treatment with doctor and specialist, he was taken off work on 7/31/09 until evaluated by specialist.
Employee was mounting up the ladder of a 992C loader to service the transmission fluid. As he approched the top step the oil hose reel unlocked, causing the hose to jerk the employee, resulting in him loosing his balance and falling off the loader.
Victim was directing miner operator who was slabbing a new longwall face. The miner bits contacted the wood of a fold out bridge mat constructed of six inch timbers. Injured was thrown against the mine roof and struck by a timber. He sustained multiple contusions of his chest, trunk, legs, and two fractured ribs.
Employee was picking up a timber out of the scoop bucket, when the timber hit him in the left knee.
While employee was hanging the bolter cable on the rib, the operator took the cable reel up and the cable pulled his legs from under him twisting his knee.
Employee was moving bags of rock dust into scoop bucket. While twisting with rock dust, he felt a sudden sharp pain in lower back.
Dumping coal on dump. Piece of rock caught his hand against the metal choker plates and mashed hand cutting some of his fingers.
Walking into mine opening & slipped on thin layer of ice, falling to the ground fracturing lt. leg.
Employee reported: He and a co-worker were attempting to install a piece of 1/4"" metal 5' x 2' in a chute located at the surface crusher area. When attempting to make the final 5' of movement, he experienced severe pain to his lower back, possibly straining his back.
Employee was going through a 3x3 mandoor in a stopping when dust flew into his left eye. He finished the shift but was kept off work due to an abrasion on his left eye.
Employee was attempting to pry a board off of a new pump grate with a chipping hammer. Employee struck the chipping hammer with a three pound mini sledge hammer, causing a small piece of the chipping hammer to enter his left hand. Please note: Employee did not begin missing work until 4/7/2009.
4X4 fell from scoop catching fingers between frame of scoop and 4x4. Cut left finger, sutures required.
EE was exiting scoop deck. As he was turning, his foot slipped, cauing him to twist his left knee.
Employee was using a hoist to disconnect a vibrating screen from a trailer, resulting in pain to his right wrist. Was evaluated by a Physician and on 4-7-09 had surgery resulting in lost time.
EE states that as he bent over to pick up welding rods, a 4' piece of angle broke loose over head and fell on EE, causing a concussion and contusions to the neck, head and (R) shoulder. EE is off work until cognitive evaluation is performed for stage 3 concussion.
Adjusted drill pod on roof bolter, was discussing work with another employee and leaning hand on canopy post when the other employee bumped the canopy down pinching tip of left middle finger. Cause was concentration and communication. Compliance with rules/regs was a factor. Miner proficiency was a factor, ppe was not a factor and mining equipment systems not a factor.
Employee tried to move the Rotap and sound enclosure by himself. He stated he had made similar moves by himself several times. This time, he said he must have twisted the wrong way because he ended up straining an abdomen muscle and a groin muscle.
Employee lost balance and fell on to kiln anchors. Suffered lacerations to the right knee and face.
Adding new drill steel to one of the company's drills. The new steel was put in place by using the boom on the mechanic's truck with a strap around the steel. After several attempts of lining up the steel and jogging the head to thread the steel, EE got his hand caught in the strap. When the head was jogged it wrapped his arm around the steel.
Injured was repairing cable when knife slipped cutting left index finger.
Employee stated that he was checking a ram push spool handle and as he pulled down on the handle he felt a sharp pain in his right elbow.
EE was riding in mobile facing outby when the mobile's brakes locked up and slid 70' into a battery motor. EE had the air knocked out of him and back of the head was burning sensation. His shoulders were in pain. EE was treated and returned to work on 2-16-09. EE went off work for injury on 2-24-09 with shoulder pain.
Employee stated he was operating a cont. Miner and turning a left handed slider. A rock fell from between the bolts and struck him on the head. He has continued to work until taken off by a doctor on 07/26/2009. Possible neck surgery for herniated cervical disc.
Injured was stepping off a ladder and stepped on a piece of concrete block, straining his right ankle. He began missing time on 02/16/2009.
Employee was changing liner at the concentrator with legs braced against the beam when the left knee rolled in resulting in a sprain of the left knee.
EE was at miner talking to miner operator when a rock, 6"" X 12"" X 60"", rolled out of rib. Rock hit EE on ankle.
The man was attempting to remove a drill steel from the roof when he pinched his finger between the drill head and the roof.
Employee was helping with clean up efforts after a storm came through the area. While in a manlift, removing bent up sheet metal from the roof of a building, he was pulling against a piece of metal and it cut through his glove into his little finger. The cut required 7 stiches. (Surgery 5/5/09-work off Misc Mail 6/2/09 #2)
Employee stepped off Cogar feeder onto loose material and twisted right knee. Employee suffered muscle strain and was placed on restricted duty.
High pressure blew a pin hole in 1/2"" hyd. hose. The employee was trying to cover the hose and it injured his right finger.
Employee was getting up from a kneeling position when he felt his knee pop.
Employee was knelt down (drinking water) and a piece of rock (about 4 lbs.), fell from the top and landed on his left hand, injuring ring and middle fingers.
Employee reported: Operating continuous miner in face area, feet became entangled in the machine's cable. He stumbled and fell across a pile of slack resulting in a twist to right knee. Worked the entire shift, called the mine the following morning, reported his knee was swollen and painful & was to seek medical treatment. Diagnosis was a sprain to his knee.
The EE stated he was walking outby in the No. 7 entry and as he came through the back-up curtains, he slipped on a ledge in the bottom and fell to the mine floor.
Hanging curtain when a shuttle car was moved pulling the cable down on his face and causing him to fall on rock.
The employee stated he was bolting top and was taking the steel out of a drill hole. When he took the wrench off the steel, the steel fell. He reached for it and felt a pain in his shoulder. He went to the doctor and x-rays were negative. He has been diagnosed with calcification tendonitis of the right shoulder.
Cleaning out the HSI to adjust liners with pry bar. He pryed up with pry bar and hurt right elbow.
The victim was drilling top to install roof bolts and was using a 12"" wrench to finish the hole and the wrench broke and struck his hand and thumb, (right hand) broke thumb.
When splitting rock, the rock came back, hitting his left hand and jammed his hand between the rocks.
Miner was lifting belt structure and felt pain in his back.
Employee alleged that he was lifting a rock with a 992G loader when the rock slipped out of the bucket causing the loader to rock throwing him out of his seat hurting his back. Dr. diagnosed strained back. Seat belt was in good working condition but was not being used.
Employee was stacking 50 lb bags at the turntable in the main warehouse. When he picked up a bag from the turntable he felt a pop in his right elbow area. He continued stacking bags for the rest of the day. The next day he complained of pain in the right elbow and went to the hospital for evaluation.
Running 16 motor grader, took a hard bounce which resulted in air ride set bottom olut causing back pain. Note: Employee came back to work the next shift and worked til 3/5/09, left job & went to doctor.
Lifting box of glue and felt ""pop"" in lower back.
Alleged back strain while changing 13lb bags in dust collector. Miner worked entire shift and 2 scheduled hours next day, did not report any injury or pain. Called leadman to report back pain on Sunday, 02/15/2009, at 2:30pm. Produced emergency room release on Tuesday, 02/17/2009, showing treatment with no recommended restrictions or missed work days.
While standing on blow pipe, his foot slipped between blow pipes and bent his toes backwards. He was carrying a candle-abra. This was approximately the fourth one he carried. He knew some of the candle-abra were wobbley.
Tripped going up steps, fell on left arm. Accident occurred on 2/12/09, did not seek medical attention until 6/25/09.
Employee stated he was driving the slinger duster down a travelway, when he thinks he hit a hole and caused him to be bounced up and hit his head on the canopy. He felt in his neck and head. Employee had prior neck surgery and the MRI showed degenerative stenosis and cervical spondylosis. Employee was taken off work on 02/20/2009.
Employee was attempting to cap off concrete block brattice when brattice fell over. Concrete block struck employee's lower left leg. Employee sustained a simple fracture to the left lower leg.
While bolting, the operator had a rock fall from the roof, striking the canopy and then it hit his right foot just behind the metatarsal part of his boot. The rock weighed 25 lbs. He was operating the helper side of the RRII roof bolter.
Injured was removing panel from c/m and cut left ring finger, he received stitches.
Two men loading pipe on truck. One dropped and transferred impact to other's shoulder.
Employee was in a shuttle car heading to miner when it hit a hole and threw his head into the canopy.
Employee operating the automatic bag filling machine and noticed malfunction of the bag applicator; he removed the guard that protects the pulley and introduced his hand to check the belts; his arm obstructed the photocell device activating the spout mechanism. His left hand got caught between the belt & pulley.
Injured was walking down the grade at the dust hole when her left ankle twisted causing her to put her weight on her right knee and leg causing pain. She was transported to Princeton Hospital. She is being treated for a hyper extended right knee.
Employee states he was driving a groung rod with a hand held tool designed for this task, when he hit himself in the head.
Employee was traveling down steps leading from an overcast when the steps fell. Employee refused medical treatment on 2/11/09 and sought medical treatment on 2/12/09 claiming an injured shoulder.
The injured miner was walkin goutside on snow, following behind another miner who was also walking. The injured miner apparently slipped on some snow covered ice, which caused him to fall, twisting his left leg and landing on his right elbow.
Employee was operating a rib runner on the 3rd North Main when he ran into a parked 2-man diesel ride. The ride was parked against the rib, in a well lighted xcut, and also had reflective material on the back.
Injured was tramming scoop backwards when he ran over board causing it to come into deck.
Employee was operating a continuous miner when a piece of roof rock measuring 4'x 2' 4"" fell striking him on the back. He returned to work on the following shift but has continued to have problems with his back. He had a follow up Dr visit on 2/23/09 at which time he was taken away from work. He started losing time on 2/24/09.
EE stepped outside the side door of shop, twisted ankle stepping off a step.
Miner was operating a mantrip. Ran over a post and it jostled him around in the cab.
Employee was removing rail from old # 18 panel. One rail hooked, had scoop operator turn scoop bucket in order to hook other rail. Bucket hit rib, caused material to fall onto employee's right leg. Taken to PMC ER for treatment X-Ray positive. Was no indication of bad rib.
Employee was reaching a wrench under a truck to another employee, when the employee straightened up, he felt pain in between his shoulders.
Mechanic was changing ripper tooth on D11 dozer. When he went to lift the ripper tooth up, he had immediate pain in his lower back.
Two millwrights were removing a gear from the shaft of a brake. The gear was heated up and one millwright held a B&O hammer next to the gear while the injured employee was hitting the B&O hammer with a 12 pound sledge hammer. As the injured swung the sledge, he felt pain in is lower right groin area, causing a double hernia.
Injury was checking clay falling on CV25 belt. He started towards spray system to check tips and slipped on wet surface. He fell injuring his left knee. Diagnosis - strain/sprain & joint mouse of left knee. He is scheduled for surgery.
Employee experienced pain in his right shoulder while mounting a D-11 dozer. Employee did not begin missing work until 2/20/09. He was diagnosed on 3/4/09 with a rotator cuff tear.
The employee was building a ventilation stopping when he bent down to pick up a block & he strained his back.

The employee was assisting a contract welder who was working in a feeder area of the plant. The injured employee was acting as a spotter in the event that the welder needed help due to working in a confined space.

Roof rock fell and hit employee on the head and shoulder while operating a motor. First day of work missed was 2/12/09.
EE ran over a large rock while operating a scraper which caused him to pull groin muscle in right leg. He expects to return to work on 02/23/3009.
EE was making a delivery to the truck shop. When he stepped down from the cab of the truck, his foot turned under him twisting his ankle.
Rib rolled off and struck employee in the back. He was working in #3 entry.
789 rock truck - strips missing. Cut strips on back of welding truck then put strips on ground & welded brackets to lift metal with boom. Finished shift and went home. That night around 11:00 pm started hurting in left shoulder and neck. Went to ARH Hospital in Hazard Ky.
Employee squatted down to loosen a bolt was unable to stand back up. Lumbar strain. Was unaware of termination until now, 12/14/09.
Employee was hanging miner cable and felt a sharp pain in his right shoulder. Hospital tests have came back inconclusive at this time.
Climbing to get into Grader, right hand slipped from the assist handle and fell into blade hitting left side back (rib). Fell off machine landing on the ground.
2 guys were using 15 ton hydraulic pin pusher to remove WH78 drag chain. The injured was holding a hammer handle and the other millwright was applying pressure with the manual hydraulic pump. Replaceable end of hydraulic ram shattered striking injured on right side of face shattering lower jaw and fracturing upper jaw. He also suffered a three inch laceration on the right cheek.
Employees were investigating methods to refeed emergency power to MCC during power outages. The employee noticed a jumper wire hanging from the bottom lip of the bucket. While using a voltage sensing probe, an arc flash occurred causing a burn to right hand of employee.

Employee was doing winter maintenance. He cut a piece of steel and was going to install it, when steel slipped and fell on his right foot, causing contusion.
Getting up from the lunch table employee complained of back pain. Was evaluated by a Physican and was taken off of work 2-10-09.
The employee was taken to the company clinic on 4/16/09 and was placed on restricted duty. The employee is alleging a continuous trauma from 10/2/07 - 10/2/08 to his back and lower extremities from continuous and repetitive motion. Employee, on 2/9/09 originally reported his back injury as a non-industrial specific injury that occurred at home.
Employee was walking down the concrete ramp, employee had his hands on the handrails at the time and stepped on some loose pellets and slipped and caught himself. Employee alleged that he injured his Lower back at this time.
Employee was climbing up to get into a piece of equipment when he slipped down and hit his side on the equipment.
EE was walking down the belt line entry to the surface and stepped in a hole and twisted his right ankle.
Victim was removing the cutting edge from a 980 loader bucket. He was on next to the last bolt when the cutting edge swung down and struck him on the top of his left foot. He went to his family doctor and was sent for an X-ray which showed no broken bones, bruises only. Missed 3 days work to allow swelling to subside.
Employee was helping hang bolter and miner cable. He was walking and hit his head on roof top. Strained muscle in neck.
While employee was placing a bulk bag on the hydrate bagger, he overextended his reach which caused a muscle stain in his lower back.
Unit 26 was replacing cracked turtle(cover plate) on 18 shovel, the shovel operator was assisting unit 26. Unit 26 noticed the operator's face /cheek had some blood on it. The EE was transported to the foreman's office and the 1st responder transported EE to E.R.
Employee had been welding and helping another welder weld on a drainage pipe throughout the shift. Employee was diagnosed with flash burn.
The employee was helping to move a shuttle car tire into place when he felt pain in his lower back. He began losing time as of 2/10/09. **Employee began losing additional time as of 7/24/2009.
Employee was barring down loose ground when a chunk of concrete, approximately 40 pounds, fell on the employee's ankles, bruising both.

Employee was going to work on the shanks of a Caterpillar 992 FEL. As the bucket was rolled over to gain access to shanks, a chunk of frozen material dislodged from within the deepest part of bucket, striking employee.
Employee was walking down a drill bench when he felt a pulling in his back, employee stated he thought he made it worse when he climbed up the ladder into the drill.
Cleaning belt with scraper when scraper was caught and pinched hands into side guard & tightening screw breaking right index finger degloving left index finger and bruising left shoulder.
While working with other employee relocating 6"" water hoses about 25' long, into a trench in Phase I, employee allegedly twisted and strained his back. Employee worked 8.0 hours the next day, and did not show up for work on 2/9. EMPLOYEE REPORTED THE INJURY TO THE NIGHT SUPERVISOR ON 2/9/09.
The plant was off for maintenance. Employee was greasing a bearing on the BCV155 conveyor head pulley. The plant operator turned on the plant, the start up alarm sounded. Employee did not lock out / tag out the conveyor. The conveyor started and pushed him into a safety pole that holds safety cables around the conveyor.
EE was carrying cement blocks and felt pain in left groin and testicle. EE went off work on 3-27-09 for a hernia repair.
Employee complained of slight pain or discomfort in Left Wrist, did not know of any work contributing factors at that time. Employee was treated at Prescott Valley Primary and Urgent care with no confirmed diagnosis.
While handling a miner cable, the employee felt pain in his lower back. Please not: This employee did not begin missing work for this injury until 3/11/2009.
I was tamping rocks into a hole. I slipped on ice and fell causing rock to fall on his lt 3rd finger resulting in fracture
Employee was using wire ties to keep chains from moving on the dredge. Employee decided to cut the wire ties with his own pocket knife. Employee placed his left hand on top of chain flat and cut toward his hand causing a laceration.
The employee had finished fueling a machine and was driving fuel truck away and stalled the engine. He rolled backward approximately 10 feet and hit the track of the machine. Employee pulled muscles in right shoulder.
The miner was turning up #6 entry, he smashed a ladder, he stopped, moved the miner off the ladder, hit the e-stop on the machine. He traveled between the tail of the miner and the rib when the machine relaxed, shifted, striking him on the hip/pelvis. Witnesses agree he was not in the red zone when he deenergized the machine before he moved toward the front of the machine.
While employee and a coworker were lifting I-beam over the top of a conveyor belt, he felt slight discomfort in his lower back. He finished his shift and that week of work but called the next day 02/07/2009 to say his back was not getting better and was going to see a doctor and would not be into work on Monday 02/09/2009 his first missed day.
EE was going to the continuous miner to get a load of coal, when he struck the coal rib with the shuttle car he was operating. He struck the canopy with his head, springing his neck.
Employee was walking beside a parked ramcar. The parked car was sitting a little out in the intersection and another car came through from the other direction, hit the parked car and knocked it sideways, causing the wheel guard to strike the victims ankle and foot against the rib.
Employee was riding in the #3 4-person mantrip traveling outby on 12 headgate's track. The mantrip operator noticed a mantrip parked on the track and applied the brakes causing him to slide into a parked mantrip. The collision caused EE to fracture 3 ribs & dislocate his right shoulder.
Stepping upon the excavator. He was holding onto the handrail stepping up on the excavator. When he stepped up and put weight on his leg his knee gave away.
Employee stated that he was checking his drill steels at the left side of the continuous miner on the 11 Left section when a rib roll measuring 4"" thick x 5' long and 4' wide fell against the employee knocking him to the mine floor. Employee stated he had pain in his lower neck and lower back on his left side.
Employee first day missed was 2/09/2009. At approximately 1700 hrs a mechanic was installing a cover for the rear axle box on a loader. The axle cover weighed approximately 25 lbs. The mechanic was sitting on the floor lifting the cover into place when he felt a pull in his lower left side.
Employee did not report injury until February 9, 2009. Employee was jackhammering between 2 mill pipe lines. Left foot slipped on loose material. He grabbed jackhammer, causing strain to hip.
EE was bolting top when the drill steel became lodged. He dropped the head and struck drill steel to free it, but was unsuccessful. Raised drill pot and engaged rotation to free steel. Drill steel was not properly seated and when is came out of hole struck his left wrist.
Employee stated that he was getting out of the cab of the roof bolter and tripped over a curtain. He fell backwards and hit his right elbow on the machine canopy and also fell on his right leg. Employee did not start missing work until 10/04/2009.
EE stated that he had been in the control room area on the 2nd floor & had reported to his supervisor that he was not feeling well. The supervisor stated that the injured mineer sat at the table for 15 minutes, got up and left the room. The injured went down the 1st flight of stairs to the landing, turned & started down the 2nd row of steps not using the handrails. He fell.
Employee had positioned his left leg over the pusher assembly trough in order to add grease to a belt alignment fitting. The pusher assembly flipper was advanced and his leg was pinched between the flipper and a conveyor car pusher block resulting into a fracture and muscle damage to his left leg.
Hanging roof tube in #2 entry, slipped and twisted back.
EE was climbing down from rock truck to check on flat tire. As EE dismounted bottom step, he slipped, falling to the ground.
Employee was walking toward a conveyor belt to help get it running after the belt had chocked down when he slipped on some ice & fell twisting his left ankle. (There was a severe ice storm 6 days prior to this accident.)
Walking to electrical building to get supplies, slipped on ice and fell on left elbow causing a bruised elbow.
Employee was operating a valve pack line and strained her back.

Employee was running dozer. Hit rock, jarred him around in the cab hurting back/leg.
Employee stated that she and witness had just finished cleaning 1 north belt at 50 block. The witness attempted to throw a broom across the belt. The broom struck a belt chain and fell onto the top belt. He attempted to reach onto the moving belt for the broom when her left hand and forearm was caught between moving top conveyor belt and a top roller causing laceration 9 stiche
The crew was in the process of packing old belt structure through a man-door. Employee and another employee were in the process of carrying a belt cluster when he tripped over a belt roller and fell. As he fell he twisted his ankle.
As injury was installing a roof bolt in the rib, he pulled the drill steel out of the hole, a piece of coal and rock measuring 48 inch long by 18 inch wide and 13 inch thick fell from the rib striking him on the left knee causing a contusion.

Injured employee was in the process of installing permanent roof supports in a brow overhanging from the mine roof. He had placed the resin grouted rod to be installed into the hole & was pushing on the rod with his hands to straighten the roof bolt. The brow broke off & struck his right hand, resulting in a laceration that required sutures to close.
While unloading a shipment of siding from an over-the-road trailer, the load shifted striking employee on top of the left foot. Employee was wearing full metatarsal boot which offered only limited protection.
Slipped and fell on left knee due to snow and ice.
Employee was removing one way grease valve when it blew out striking his right hand.
While cutting plastic shrink wrap from a pallet of rock dust. Injury was holding the plastic wrap with his left hand and cutting with his right hand. In doing so he cut his left hand.
Employee was drilling with jackleg. Jackleg kicked out and the handle hit left knee and hyperextended it.
Helping the mechanic work on push out lever on a Ram car, while prying down on the arm, he lost his balance and fell into the Ball hitch on a 4 wheeler buggy striking his right knee and the right side of his back on the hitch. He started missing work 2/19/09 and missed 2/20/09 and reported back to work 2/21/09.
Operator stated that the 773-B rock truck skided into the berm, then comming to a rest on the left side of haul road into a mud hole. He was taken to the Hazard ARH Hospital. They did a CSAT Scan on him, the doctor said that he had a strained lower back and should be able to return to work in a couple days.
Walking to mine office, slipped and fell on ice.
Correction to E- Document 781351 EE was to report to work on Thursday night , but was unable to because his knee was bruised from previous days Accident.
While climbing embankment, worker stepped on a clot of loose dirt causing him to twist knee.
Employee was changing the grate at the end of the Rotary Dryer. He had removed all of the bolts. The grate normally stays in place when the bolts are removed. This time it came loose and fell striking him in the nose and upper lip.
EE was bolter helper with the track bolter. Several pieces of rock fell, striking ee on the right side of head, shoulder and back area.
Employee was hooking a chain, slipped on the ice and fell on his left shoulder. Note: Did not know the employee lost time until 2/16/09
Employee stated he felt pain in his back as he closed the freight elevator door.
Individual was stocking parts in the warehouse when he felt pain in his lower left side. Individual was taken to see a doctor, had a surgical consult on February 9th, and underwent surgery on February 10th for an inguinal hernia.
Employee stated he was walking around a power box when he apparently tripped over a 3/4'' water hose. He stated he fell to the ground but tried to catch himself with his hand. His shoulder was dislocated. Investigation showed that approx. 6 mos. ago he dislocated this shoulder at home. Doctors report stated this could happen again, will need surgery. Occup. Illness
EE was passing through steel sliding doors. EE could not get the door closed, so he shifted his weight to close the door and felt pain in the right knee. EE went off work on 3/27/09 for knee repair.
The miner was filling the water car, the fill hose got loose and the miner fell onto her shoulder.
EE was re-railing a mine ballast car with a Royal Hydraulics air pillow. When releasing the air pressure the bag kicked out striking the EE in the left jaw area. EE suffered a tripod fracture to the jaw.
Employee was pushing in a shot, when some debris fell from the highwall striking the dozer window. The glass from the window cut his arm and some glass went into his eyes.
Two employees were in the process of changing a broken sheer pin in the sprocket of the conveyor chain of a conveyor belt feeder. The injured employee had placed the new pin in the sprocket & asked the other employee to turn the sheer hub in order to line up the hole. During this process the injured employee's middle finger received a laceration that required sutures to close.
EE was removing bearing from shaft of Rotex gear box when the bearing broke. A piece of metal entered through top of left thumb and lodged in left wrist. Surgery will be scheduled to remove the metal fragment.
EE had picked up two buckets of bits to take to the shear and as he turned to walk away, he felt a sharp pain in his right side of lower back. He did not cease work at that time. He later went to the doctor and was taken off work on 02/17/09 until he had an MRI. On 02/24/09 he had an MRI and was diagnosed with a bulging disc in his lumbar area. He remains off work.
Subject was carrying a yippi post on his shoulder. When he tried to drop it to the floor it caught on his clothing and it dropped straight down and hit his right foot.
Employee smashed finger in muck bucket.
Twisted right ankle as employee was climbing down the extention ladder, about 4 rungs from the bottom, when his foot rolled. Ladder was reported as being wet.
EE injured neck and back from a fall while loosening the lids on the rockdust tank.
Employee was operating a Cat 777 rock truck. While being loaded, the end loader operator dropped a rock out of the loader bucket. This rock hit the bed of the truck and jarred the employee.
Employee exiting backhoe, as he was stepping to the ground he slipped on ice, falling and hitting his left shoulder and right thigh.
An employee was attempting to dislodge a large rock that was wedged between the push arm and track adjuster on a D11 dozer when he experienced a spain/strain injury to the bicep area of the left arm. Medical evaluation of the injury determined that the bicep tendon had ruptured requiring surgical repair.
The employee believed he had lowered the dump body on the 35 ton haul truck. He hit the conveyor, which fell onto the back of the truck. When the truck hit the conveyor the truck tilted on back wheels until it broke free from the convoyer causing the truck to land back on all 4 tires.
He was mopping the B Mains belt when the mop was caught by a return roller causing a strain to his left wrist.
EE was removing a chute, slipped and fell and fractured his left foot.
Miner was winching a relay bar over the stage loader it became fouled. The miner attempted to dislodge it. When he did, it slid around and struck his lower leg.
Employee was struck by a rock that caused a fracture to his left leg and left index finger. Started missing work 2/3/09.
Pulled back muscle, during twisting motion, while using a grinder.
While loading roof bolt supplies into scoop, the scoop operator dropped scoop bucket on employee's right foot.
The employee was working on the outside of a hopper that became jammed with material. He was using a pry bar to clear material from the hopper (were it discharges onto a belt). The employee had the bar resting on his shoulder using his legs for power. Later in the day a pain developed and continued to get worse. He went to the local E.R. & was diagnosed with tendonitis/strain.
Employee was moving up the mine phone when he tripped and fell against the rib striking his shoulder.
EE had loaded some monster roof plates on the deck of a Mini-Trac to transport them to a roof bolter. As he was traveling down an incline, the monster plates slid into the deck, causing injury to his right leg and ankle.
Employee and a co-worker were moving a vulcanizing transformer into position to be energized inside a building. Employee's fingers were pinched between the transformer and a fire extinguisher mounted on the wall. Lost time injury as of 09/16/09 due to surgery.
Employee reported: When walking across section, stumbled and fell onto his left shoulder. Pain and discomfort continued, 6 weeks after event received medical treatment. In late June, MRI was taken, determined he a torn rotator cuff in the left shoulder. On 7-23-2009 surgery was performed to repair the tear. Became a lost time reportable injury on that date.
EE was standing in mud, stacking blocks. When he turned, his left foot slipped and he heard a pop in his right ankle. X-rays revealed fractures to the right ankle. Lost time injury.
Employee was pulling ""J"" hooks from the loadout tunnel chute. he turned to the right and lost his footing, causing his right ankle to roll in. resulting in a Sprain of the right ankle.
He was fixing machinery slipped and fell backward. He fell about 10 feet and landed on his heels. He injuried both heels, left knee and lower back.
Employee was working on an engine installation for a Highwall Miner Generator Set. We suspect he inadvertently hit the door release button with his leg or foot and fell to the ground. He possibly has a broken left wrist and right shoulder.
On 2-26-09 EE was struck by a piece of coal which may have aggravated a previous neck surgery. EE went off work 7-13-09 for surgery. After a review of all of the doctors reports it has been determine that the second surgery was required because of an aggravation. The difference between the accident date and reporting date was pending the final review of his surgery.
Employee was repairing the water line on the crusher when he slipped and grabbed the guard straining his back.
Employee filled sandblasting machine and was bringing it back to shop when he slipped and fell. The machine landed on his foot.
Performing 1000 hours service on 345 excavator. EE attempted to climb on to the machine and due to rainy & muddy conditions, his foot slipped off the step causing him to slip and twist his left shoulder while holding onto the handrail. Pain in shoulder got much worse on 3/2 and ee sought treatment.
The employee was operating scoop hauling timbers to section and the band broke allowing timbers to roll and scatter about the mine floor. He stopped the machine and was dismounting, getting his foot caught twisting his rt. knee.
RAINY, SLIPPERY & MUDDY. EE STEPPED DOWN OFF OF LADDER IN TIGHT QUARTERS ONTO UNEVEN FLOOR, TURNING ANKLE UNDER RIGHT LEG.
Employee was putting 4"" pipe together and when he pushed the pipe, he felt a pop in his left arm. Reportable lost time injury 04/13/09 due to surgery.
Employee was driving a Getman when he hit the rib, causing the rib to break and strike him on the neck and shoulder. Lost time injury.
Employee was lifting pile of roofing shingles.
Injured was traveling through a mandoor and felt pain in his lower back and left leg.
changing crowd ropes on shovel, forklift hit employee on head, pushing hard hat over ear and cutting ear, requiring stitches.
The employee got off the John Deere Skid Steer, and stepped between the bucket and frame when his left foot slipped out from under him, and he twisted his right knee. He requested to see a dr on 3/3/09 and was placed on light duty
Injured employee and co-worker were carrying part of an airslide up a small set of stairs, when he felt pain in his knee.(This injury did not became reportable until EE started to miss work on May 4, 2009 due to surgery Misc Mail 5/8/09 #2)
Employee was reaching into a hopper to remove boulders, when he felt a ""pop"" in his right lower leg.
Employee was installing cribs when a crib was thrown, striking his little finger. Finger was fractured and employee has undergone surgery to pin the finger. Lost time injury.
Employee was working an overtime shift and was returning to the Tipple to eat lunch. He tripped while he was walking away from the truck. He heard his ankle pop and was taken to the Hospital. He has a broken right ankle.
Employee was walking on a blacktop surface, when he went to turn his leg turned but his ankle didn't, causing an ankle sprain to his right ankle.
On 2/17/09, employee was operating a Dozer (equip# 2550) to remove a toe (rock protrusion) that required more time and force than normal to remove. At the end of the day employees back was sore from all the jolting of the equipment. Employee worked a full shift the next day and his back was still sore. He reported the injury to the supervisor after his shift on 2/18/09.
Employee was going down hill when Tram pedal on #176 ride stuck. Personnel Carrier hit rut causing steering wheel to spin and knob hit employee on right forearm. Knob broke off wheel.
Employee gathered 12 1lb sample bags of DE and put them in a garbage bag. She put the bag over her shoulder straining her upper left scapular area.
The miner had previously inspected and barred down the heading before drilling. An unseen rock fell from the rib onto the bench and slid down striking the driller in the back. The driller then walked to a nearby worker and they determined that he should go to the clinic for further examination.
Employee tripped over an object that was used to keep open a service door and claims to have injured the foot.
While lifting winch cable to unhook from railcar, cable twisted and injured lower back.
Employee was bolting when he slipped, causing the jackleg to fall on his right thigh, fracturing the right femur.
On Saturday 2/14/09, EE slipped & fell while walking on trommel deck grating on Maxvill Wet Mill. EE was transported to a local hospital. EE sustained a torn meniscus & ligament as a result of the fall. Prescription medication was administered. Injury has been classified as an OER/RWC. Amendment: Reclassified as OER/LWC due to surgery on 3/11/09 (1st day missed work).
Repairing a conveyor belt and the board in which was supporting the belt slipped and caused EE to fall 5-6' on the ground.
Dump operator attempted to remove a rock from feeder without turning off the feeder. He cut his hand.
Employee was loading a container into a Bronco when the door blew shut. He set the box on the ground opened the door and picked up the box. He felt something sting in his torso and assumed it was a muscle pull and did not report the accident. Over a period of 2 months the condition did not improve. He reported the accident on 4-10-09 after discovering that he had a hernia.
Employee was shoveling clay material from under a blending hopper. As he was dumping the clay from the shovel he hit his elbow on a support beam. On 3/5/09 he was taken to doctor for fluid retention on his elbow. The doctor removed the fluid from the elbow.
Employee was assigned to work on the conveyor belt. As he was traveling (walking) in a x-cut that contained muddy gob on the floor he slipped on a 3"" water pipe and fell onto his buttock. He was taken by ambulance to a hospital and treated for a contusion.
Employee was team lifting a small portable generator and strained him self to point of causing abdominal pain.
STEEL PLATE FELL ON INJURED EMPLOYEE'S ANKLE WHILE PERFORMING MAINTENANCE CAUSING FRACTURE.
Injured was riding as a passenger in a 9 ton mantrip facing opposite the direction of travel. Another employee inadvertently left a track swith thrown for a track chute. The mantrip traveled into the chute and struck a parked duckbill motor. Injured was jostled in the mantrip and strained his neck.
Employee was descending the driver's side ladder of A Cat 785 B haul truck when he lost his footing on a ladder rung and fell backwards twisting his right ankle.
Putting skirt boards on piggy back conveyor of surge bin. Was reaching out and lifting in an awkward position and pulled something in his back.
Employee was cutting a flap of rubber with a knife. The knife slipped and cut his right leg.
Supervisor noticed employee was disoriented, he then noticed that her hard hat was lying on the ground. She was transported into the hospital.
While mining, he was trying to step up onto the bench by pulling himself up with a rope, he felt something in his shoulder pull. Had pain and could not lift his arm.
Mechanic was working in a access hole on a D11-R dozer repairing an implement pump. He was in awkward position loosening a bolt. He pulled a muscle in his left shoulder.
Employee was bending a roof bolt when a small piece of drawrock fell against t bar causing him to jerk back. Sudden motion caused severe pain in lower back and testicles.
Pumping water out of permanent barge dock. Pump hose was inside barge with lid/hatch open. Stepped in hole with left leg. Over the next couple of days, experienced pain in right leg from shock/strain. Followed up w/occupational medicine on 2/24/09. MRI ordered, surgery 3/11/09.
The 7-foot crusher would not start. Employee took a meter to check the fuses. While he was checking a fuse, one of the other leads got close to another phase, causing a short. The meter was too small for this voltage and burned his hands and face.
The EE had to walk sideways through a 23 inch walkway to go to the maintenance shop. The employee was side-stepping to the right. As the EE was free to sqare up his body, the next step is where the EE heard and felt the pop in his right foot. The EE fractured his right 5th metatarsal. A splint and restricted work was the outcome, thus resulting in the MSHA reportable.
Employee was carrying a 50 lb object and stepped off a 7"" curb. When he stepped off, his foot was not in full view and he fell with his entire weight onto the right ankle, resulting in a fractured fibula. Employee was unable to work the next scheduled shift.
After starting the blade the employee stepped on the tire to get down. The tire was covered with frost & his foot slipped. He fell & landed on his hands, which jammed his elbow. He felt that everything was fine at the time but the elbow got worse. He sought medical treatment on 02/13/2009. An x-ray revealed that the end of his elbow had a small fracture.
Employee was exiting the skidsteer, he was standing in the bucket, he turned right and went to step over the side of the bucket (about 14"" high). He hooked his boot and fell on his left side.
EE was lifting a cross-over duct work onto M15 miner. The EE felt pain in his lower back. He finished the shift and worked several days after. On 2/16/09 he went to the doctor and doctor would not release him to return to work. On 2/17/09 this became a lost time ""Reportable"" incident. The EE had tools and equipment that was there to help lift, but chose not to use them.
Employee reported pain in lower back over the last few months.
