#3 hoist tripped on an ""EMS Fault"". Electrician had to be called out of the mine to check and reset the hoist. Hoist was back in service at 4:15 am. There were still two separate escapeways during the incident.

Roof Fall, Abandoned 7th Sub Panel North (OID #3) off the 2nd Main west entries. This is a continuation of a previous roof fall. Approx. 20' long x 6 1/2' high and 18' wide.

A roof fall occurred at outby spad #1499 and was discovered at 1:15 am. The x-cut fell in at #5 entry through break over to start of #6 entry. The length of the fall was 6 x 18 x 6. The fall will not affect the passage of men. No injuries occurred. All access to the fall was timbered and barricaded.

A roof fall occurred on the 3rd Northeast Mains. The fall is in an area of the mine that is used to travel to the bleeder system. The fall is in the #6 entry one break inby survey station no. 685. The fall is approximately 16' wide x 20' long x 5' thick. The fall was found during a weekly exam and reported to MSHA.

Fire boss run on midnight shift discovered roof fall. Fall occured between day shift ended and midnight shift started. No one working underground pm shift. Size 11' long 10' wide 61' high main intake no equipment or employee involved.

An unplanned roof fall occurred on 60-2 transfer belt, between 2&3 crosscut at spad 0270. The fall measured 8'X16'X30' on converyor belt and total fall was 60'X20'X4'. This area was developed in June 1997 and supported with 42"" resin bolts. A fall occured previously in the area in Aug 2005. Ventilation was not interrupted and passage was not hindered. Will be rehabilitated.

Unintentional fall of mine roof located in the belt entry at 48 cx on the north west mains section in the #5 entry. Distance from the face is 600 ft. Ventilation to the section is uninterupted. The fall measured 20' x 20' x 14' high.

A roof fall measuring approximately 7 feet by 18 feet by 30 feet was found near survey point 11733. There was no change to the ventilation or travelway.

A roof fall was found on the 003 Active section at our mine The fall is located in an intersection break outby spad 2475 in the #4 entry. The area fall is approx. 70' long 20' wide and 5 1/2"" high. The area had been dangered at timbered off.

The chippy conveyance tripped on the way down, 326 feet beow the collar, in a controlled stop with two men in the conveyance. The problem was identifed to be a cotrol transformer winding. No persons were injured. The 7000-1 is being filled out to terminate citation issued.

An insulator on a High Voltage line failed and shorted to a lightning arrester throwing sparks into dry grass below the pole. A grass fire ensued that required the assistance of the local fire department to bring under control. MSHA was called and the fire was under control within an hour. About 3/4 of an acre burned.

A small roof fall occurred on #2 unti, (mmu-002-0) crosscut 15, #2 entry. The area was barricaded off, timbered and cribbed. Three roof bolts were pulled from the roof at the 4 way intersection.

A fall was discovered in the No. 1 entry of the 1-left panel off 1 East marker at survey station No. 6600. The fall measured approximately 25' long by 7' thick by 17' wide. There were no injuries or equipment involved. The fall was approximately 650' outby the MMU 001-0 and will not be cleaned up.

Employee was sweeping Lost Creek road when he was hit in the rear of the sweeper by an ATV driven by co-worker. Police report was filled out.

Our examiners found a roof fall during their weekly examinaton of the 3 Left 4 Main North (5A) panel. The fall was located at crosscut 16 between entries 2 and 1. The fall was approximately 50' long, 18' wide and 7 feet high. The fall did not extend into either intersection and was confined to the crosscut. The fall was located adjacent to the examiners route of travel.

A fall of roof occurred in the South West Mains at cross cut # 26, # 5 entry. The fall was approximately 30 feet in length x 20 feet in width x 10 - 12 feet thick. The area was supported with 6 foot fully grouted roof bolts.

During a 60 day inspection it was discovered that there was a kink in the slope car hoist cable. The cable was cut off, re-socketed and re-attached to the car. The slope car was unavailable from 10:00 a.m. until 2:30 p.m.

Rock prevented clam shell from opening on ore skip of Hoist #3. Attempts were made to notify MSHA, but supervisor was unable to leave message at ""800"" number. No mine employees were trapped, two ways of escape were still available at all times while miners were underground.

An unplanned roof fall was discovered in the Main North section in #7 entry at the +1495' mark. The fall was immediately called in to MSHA and all proceedures have been followed. We at Royal Falcon have moved the 001-0 section into the 1st West Panel and have abandoned the Main North.

#3 hoist tripped when the #4 west brake did not actuate. The brake solenoid was replaced and the hoist was back in service at 10:05 AM. There were still two operable hoists/escapeways throughout the incident.

2 South Section, Roof Fall occurred #4 entry x-c 42 in a 3 way intersection 25'w x 30'l x 18' hi. A x-c was turned just inby a known fault area. Area bolted with 8' tension rebar full grout bolts.

Unintentional roof fall 50' long x 20' wide x 3' to 5' in thickness. The fall is located on 8 left panel #14 crosscut and #8 entry.

The #1 elevator was taken out of service for the scheduled 5 year service required by ANSI article 17. The elevator was down for a period of time exceeding 30 minutes all affected escapeways were re-routed no individuals were on the elevator at the time. The elevator passed all tests and was placed back into service @ 3:00 pm.

IGNITION: The Continuous Miner was cutting in the No. 1 Face on the No. 8 Section. They were cutting in middle of face and cutting bottom rock when an ignition occurred. It lasted approximately 20 seconds. It was put out with a fire extinguisher.

The fall occurred in the #9 seam in the 2 Main North, #3 entry between crosscut #3 and #4. Roof fall of about 50' long 18' wide and 6' thick. The area was supported with 6' tension rebar roof bolts.

Termination on second set of hoist contacts broke loose from vibration causing hoist to malfunction. The mine was evacuated at 11:33 pm and completed at 11:47 pm. The hoist was repaired and tested and miners returned underground at 12:06 am.

During a normal inspection of the escape shaft, a loose guide rope was observed. Miners were evacuated from underground until the guide rope was repaired. The problem was corrected by 11:35 am.

A fall was discovered in the 3 Left off 1 North marker area of the mine approximately 50' inby survey station No. 6262 (No. 6 entry). The fall apprears to be approximately 30' long by 8'+ high by 20' wide. There were no injuries or equipment involved. Ventilation was not affected. The fall will not be cleaned up.

While pushing coal on the #4 Clean Coal Stockpile in a D10 Dozer, the dozer was pulled in a cavity. No injury occurred. The dozer and member were safely freed at 5:46 AM.

(A) section experienced an unintentional rooffall on the active pillar section pulling 3 rows of roof bolts. The fall measured approximately 15ft. wide by 6ft. high trapping 2 Mobile Roof Supports.

The continuous miner on the 002 section was cutting the 2 left crosscut into the #1 heading 120 ft inby SS 863 when it cut into old works. A hole app 2 ft x 4 ft intersected into the old works. No low O2, Methane or water was encountered during the hole through. Personnel was removed from the mines with no injuries.

The two explosion vent for the #1 kiln coal mill system opened due to a coal dust ignition inside of the #1 kiln booster fan/ductwork. A Hot Spot was found in the bottom of the fan housing.

An unplanned roof fall has occurred in the No.1 unit, No.2 entry one crosscut inby spad No. 2329 the fall measures 90' in length, 19' wide , and 5/6' thick.

EE was welding side rails on belt. Suspect piece of hot slag went under skirting at feeder hopper end. After welding crew was on site 40 add'l min. cleaning up & no sign of fire. Shift ended 6:45 pm. Slag evidently smoldered & started fire on skirting. EE usually runs belt but since was end of shift, he didn't. Damage consisted of burning up a belt & some rollers on conveyor.

A roof fall occurred on the 001 section in the face of # 5 entry inby survey spad 61814. The fall measured 18' long, 6' high, and 18' wide. 5' torque tension roof bolts was the support in use. The fall occurred due to layers of coal and rock above the permanet supports. No injuries occurred as a result of the fall, equipment operations was affected

A roof fall was found in a neutral entry along the # 8 belt 700' outby the working section at survey spad 61614. The fall was 80' long, 19' wide and 10' high. 5' torque tension roof bolts were in use. No injuries or operation of equipment was affected

A roof fall was discovered during a preshift of the 7th South panel. The fall was located at xc 8, #6 entry. The fall measured approximately 60' long, 18 to 30' wide and 7' high. The fall was located in the secondary escapeway. The escapeway was relocated around the fall.

Unintentional roof fall discovered the above date and time. Fall location is: Old 16 zone, No. 5 block, No. 5 entry, 1.5 bloccks inby station 3554. Fell in entry between intersections. Location mined on or about 10/30/08. Previously supported with 6. full grout resin bolts. Fall measured approx. 16'L X 17'W X 7'H. Approx. 240' overburden. Abandoned area.

While spinning a cable bolt to mix the resin, the plate was in contact with the ray mat, resulting in a frictional ignition of methane. The small flame was immediately extinguished with a fire extinguisher.

A roof fall was found at Right Fork Submains in the #5 entry between brk. 38 & 39, the fall was approx. 7' high, 20' wide, and 60' long, this area is located in a worked out area and in the process of being sealed off. The area has been cribbed and dangered off.

At approximately 7:00am an employee called out to another and said he had some top trying to fall in and he was cribbing it off. The top fell in at about 7:30 am. We finished setting cribs and began to move section.

At 8PM CROs alerted of main baghouse fan trip via PLC PC cntrl sys-2 plnt electricians responded to check cause-in opening E. door to Main Baghouse MCC electricians observed room filled w/black smoke-notified shift super who called 911. Electrician's discharged 1 fire ext.& pulled power to the bldg-Forks Twp FD arrived on scene w/in 10 mins-fire extinguished by 9PM. Cause TBD.

A roof fall occurred in the No. 3 entry, between survey stations #142 and #147 measuring 40' long X 20' wide X 6'-9' high.

Roof fall occurred on the North C Section, 7000ft outby the working face. There were no injuries and no ventilation was disturbed. The fall measured 18ft x 18ft x 5ft in the #1 Entry.

An unintentional roof fall occured in a 3-way intersection on the 1st East return in the #11 entry at station 34+30. The fall did affect ventilation so the unit was shut down and the stopping line re-routed. The area had been bolted with 6 foot roof bolts. The fall measured 24 feet x 20 feet x 10 feet.

A fall of roof occurred in the 1st Right, entry # 11, station # 7+25. The fall was approximately 30 feet in length x 40 feet in width and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.

There was a roof abnormality in the cross cut 2 to 1 that may have contributed to the failure. There was additional support installed in this area before the failure.

Discovered roof fall at 8:50 A.M. in intake, #7 entry, 001 section, 12,000 ft from face. Fall was 20' X 20' X 6' high. No equipment was involved, vent not affected. Area will not be rehabed, both MSHA & OMSL were notified and investigated.

An unintentional roof fall occurred in the KY Ridge Mains in the #6 entry between Xcut 26 and 27. The fall is approximately 40' long, 20 ' wide and 7-8' height. The fall does not effect the passage of miners or the ventilation system for this area. This area is part of an inactive area of the mine on the intake side and is examined weekly.

A roof fall app. 125 feet in length, 30+ feet deep and app. 30+ feet in height occurred on 4-13-09 at No.3 entry, one break inby station # 934.

Roof fall on 006-0 section. The fall was across from spad #50207. The fall was 19' wide, 20' long and 7' high. The fall has been dangered off and supported as per roof control plan.

Mine examiner was making an examination of the Byrd Mains left return & observed an unplanned roof fall at an intersection 1 cross cut inby survey station #1346. Travel or ventilation in this area has not been compromised.

The weekly examiner was conducting his examination & came across a roof fall located on the 6th Left Panel in the #3 entry at the AB Seals inby old F Head in the return area. It fell from the intersection to the face. Approximation of the distance of the fall is 70' long & 5' high. This area has now been DANGERED off to keep anyone from traveling into the area.

An unintentional fall occurred in the 2nd main west Intake escapeway. The fall was 20' long by 19' wide and above the anchorage of the 6' tension rebar bolt. This fall was timbered off.

A belt person found a roof fall located on the 3 West A travelway at crosscut #9 in entry #7. The fall was approximately 30' long, 20' wide and approximately 8' high. The fall was a continuation of a previous fall. The area had been timbered but it overrode the timbers and fell across the travelway.

Rock fall at bk-27 on 6 b belt fall is 16' long by 18' high in the #3 entry it blocked the track entry it was reported to the MSHA hotline it will be cleaned and re-supported. MSHA did investigate the area.

A roof fall was discovered in the 3 north3 mains, crosscut 7, #4 entry. The fall measures approx. 20 ft long, 17 feet wide, and 7 feet, thick. The area is supported with 5 foot fully grouted resin bolts.

When shift started, the hoist indicated a ""heartbeat fault"" in the controller. Connections from the hoist to the monitor were cleaned and the monitor was rebooted. No people were in the mine at the time of the incident. Hoist was operational at 11:15 pm.

After lowering in the tailpiece for the 5 Right longwall setup, the counterweight on the A Shaft Man/Materials cage came out of the guides. This caused man cage to be out of service over 30 minutes. Production was ceased on the longwall and the skips were setup to transport men in case of emergency underground. Shoes were adjusted, hoist back in service @ approx 12:15pm.

The # 1 elevator was out of service for a period of time exceding 30 minutes @ 10:18 AM due to a broken cable on the retiring cam. The cable was repaired and the elevator placed back into service @ 11:55 AM. No individuals were on the elevator at the time and the escapeway was re-routed.

A fall of roof occurred in the South East Mains, entry # 2, station # 9+80. The fall was approximately 40 feet in length x 20 feet in width and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.

A roof fall occured in the #9 seam, in the 2 South Panel off of the 2 East Panel, crosscut #20, between #1 and #2 entries. The fall measured approximately 35'x18'x5'. The area was supported with 5' fully grouted bolts.

The section foreman on 003 section was making an exam of the n 0/5 face area when saw a crack in the face. He checked face area and detected a small amount of cha or methane and low oxygen. He examined the n0/6 face area. found small crack in the face. detected low oxygen and low contrentration of cha. withdraw employee. notified msha.

A fall of the foot occurred in the 1st South East Sub Mains (# 4 Unit) Inby the tailpiece in the # 1 Entry throught the x-cut to the # 2 Entry.

A fall occurred in the first north west submain, #6 entry, spad 31+70. The fall is 20 feet long, 20 feet wide and unknown height.

A roof fall occured in the #11 seam, innerseam slope panel, #1 entry approximately 235' from the center of crosscut #27. The fall measured approximately 25'x18'x15'-18'. The area was supported with 6' tension rebar bolts, bacon straps, wire mesh panels, 12' cable bolts, 15' mega bolts and truss bolts.

The belt conveyor that transports quicklime to the pebble silos caught on fire. It may have been caused by the lime coming from the cooler feeders was too hot. The SLAM process was applied, the fire could've been put out faster, due to the location of the belt it was decided to not put our miners in harm's way. The fire dept. put the fire out in less than 10 minutes.

A roof fall occured in the #9 seam, 2 West Panel off of the 1 North Panel in the #3 entry in the intersection of crosscut #47. The fall measured approximately 35'x18'x10'. The area was supported with 5' fully grouted bolts and 9' mega bolts.

Fall took place back in a return area that has been standing for approx. 1 year. The fall area was found in 3 to 4 x cut. There were no signs of area working on my weekly run.

Mine neighbor had a controlled burn that he lost control of. Fire spread across line to mine property where a stack of junk loader tires caught on fire. Fire took more than 30 minutes after time of discovery to extinguish.

Drill operator was drilling blast holes. After breaking a drill steel, one had to be put back on. While adding the drill steel onto mast, the drill hammer was turning. When the striker bar of the hammer threaded into the drill steel it began to rotate wrapping the operators glove, sweat shirt & eventually his arm around the drill steel causing amputation of right arm above elbow.

EE was attempting to grind a burr off of the end of a 1/4"" x 1"" x 8"" piece of metal. When the EE went to feed the material into the floor grinder media, metal kicked & left hand was turned & pulled into grinder media lacerating left thumb. He immediately let go & pulled hand away. He was taken to hospital where they amputated thumb to 1st knuckle.

Haulage Truck operator was cleaning mirrors of truck during rainy conditions. While cleaning the driver side mirrors she slipped and fell 7 ft to the ground landing on her feet. Her right foot was injured.

Injury was hanging a miner trailing cable. As he was doing this, the miner pulled the cable tight, pulling the cable down striking him on the neck and back driving him to the ground.

turned 992 loader over into a pit. He was a contractor for Bates Contracting & Construction Inc. They reported the accident to MSHA. I got a violation issued on 7-9-09 to file an accident report on incident also.

Employee was driving in diesel mantrip when she drove through bulk dust that had been placed on the roadway. The rockdust flew up & obscured vision. She hit the coal pillar with the vehicle. She sustained a cracked orbital socket on the right side.

Welding slag got between his boot and leg, burning his leg on or about 4/8/09. Employee treated it with a burn ointment at home, no injury was reported to a supervisor. The following week his leg and knee began to swell. Employee went to the hospital and on 4/16/09 the doctor said the swelling was from infections in the leg due to the burn.

Injury was placing resin in a hole drilled for a roof bolt when a rock fell from the roof. This caused a fracture to his right arm. The rock slide down toward the operator. Our assumption is the rock hit his right arm and drove it into the drill boom resulting in the injury.

Employee was moving materials on a flatbed maintenance truck. Employee over stepped into the hitch frame causing a contusion on his left shin.

Emp. took a load of rock dust in with the track mounted rock dusting machine attach. to rock dust belt lines. He went to get more rock dust arriving at Bratt. No. 65 on No. 2 belt line, one of the steel tracks broke-and one of the ends raised up & caught bottom of the rock dust mach. & the emp. was forced into the side of the operator's compartment of the tram.

Emp was rerouting the miner cable on the miner. He lifted the miner nip onto the back of the mantrip and felt a sharp pain in the center of his stomach. Went to doctor on 4/30/09 and was placed on restrictions.

Employee was getting off his excavator when he noticed there was mud on the step. To avoid slipping from the step he jumped from the track approx 3' landing on his left heel, which in turn caused pain in his back and leg.

Employee was driving a maintenance truck down the quarry road when he lost control of the truck & it flipped. He was trapped in the vehicle until emergency response personnel retrieved him. He received stitches in his ear & was bruised. He was seen at the ER & discharged on same day. He was instructed to not return to work until results of next checkup on 5-13-09.

While repairing chains on a 992G loader tire employee was holding a bar that was being hit with a hammer and a piece of metal from the hammer or bar chipped off and struck him in the arm.

Installing pin in the bucket of a front end loader using a 10# hammer. When he applied force, he felt pain in his right shoulder causing him to drop the hammer. He was then placed on restrictive duty pending MRI. Results of MRI revealed arthritis. GA could no longer accomodate restrictions because of the indefinite nature of his condition. Last day worked was 6/8/09.

A mechanic was attempting to adjust the governor on a welding machine. As the mechanic was loosening a nut on the governor linkage, the wrench slipped off the nut and the fan struck the mechanic's left hand third and fourth fingers.

Employee stated that he was stepping off of dozer. As he stepped onto the ground, his right foot landed on a rock and caused him to turn his ankle toward the outside, resulting in pulled ligament.

EE was moving shovel parts with the forklift and the parts fell off the fork. He then got off and lifted the parts back onto the forklift. Later he felt stiffness and soreness in the knee. He mentioned he has had some knee problems with the left knee and thought by favoring his left he was put more stress on the right knee

While working on a shuttle car, employee stumbled on loose rock and fell on left wrist. Employee fractured left wrist. Surgery required.

He was taken to the hospital for treatment. Upon inspection of the area, the ground apppears relatively flat. The weather was sunny and dry. The ground may have been wet from spraying the bucket out.

EE was standing in a jon boat attempting to place the dredge pwr cord. The wind caught the front of the boat pushing the boat away from the pontoon causing the EE to loose his balance and fall into the water. The initial diagnosis was a shoulder separation. On 05/19/09 EE was told he has a fracture of the glenoid structure that may require surgery.

EE prepared to lift the radial sucker belt for repairs to be made on legs of stacker. When he started lifting stacker the turn table on the crane that sat on the international truck sheared the bolts and the crane hit injured as it broke away.

Employee slipped on a loose rock and fell to the ground injuring right knee going to change house at EOS. No medical treatment.. surgery 11/18/2009. Pre-existing knee injury in 2003. Not clear if the new injury was new -no MRI performed for April 2009 event. Post-Op notes received on December 30, 2009 noted medial meniscus torn during the April incident.

Using a sledge hammer to drive wedges and felt his back pop. Went to Dr., Dr put on light duty for 3 weeks. P.T. for 3 weeks as well.

Employee was removing plugs in preparation for loading blast holes. After he had removed one of the plugs, he turned on the ""chip pile"" to step down onto the roadway. As he stepped, his right ankle rolled. Employee reported that his bifocals contributed to poor depth perception as he stepped off the chip pile.

Employee was installing a conditioner onto float platform. Handrails had been removed for access of conditioner. Employee lost his footing while removing a 4 inch pvc pipe and fell approximately 6 feet to bottom floor. Head struck concrete floor causing concussion and head injury. Employee was working 5 feet from edge of platform which was clearly marked with caution tape.

The employee was changing the operator's side feed cylinder on a roof bolting machine. A come-a-long hoist was hooked to the machine's boom and a roof support plate. The hook came loose from the roof support plate and the boom fell catching his arm.

Employee was standing behind other employee who was using a pnuematic hammer. Employee bent over and a piece of material came between the top of his safety glasses and struck him in the left eye.

Employee was crawling in # 2 under grate compartment of # 1 cooler when he felt a pop in his left knee and a sharp pain. He crawled back out and could not put any weight on that leg.

The employee was walking in the section when he took a wrong step on his leg and wrenched his knee. He was taken to the hospital and released with a miss diagnosis of nothing wrong. The employee was released and came back to work later he recieved an MRI when his knee was not getting better and found significant damage to his knee.

Employee states he crawled under conveyor frame work to reset a string line, when a co-worker hit the leg of support jack with a hammer to align the frame. The east end of the assembly fell approx 2 ft, striking employee in lower back, left side. Employee missed 4/14-4/18. Returned to restricted duty 4/20/09.

Employee was changing out a bit on the miner with a bit wrench when his right thumb got caught between the wrench and the bit resulting in a broken thumb.

EE slipped as he was descending the rock bin staircase and pulled his left shoulder trying to catch himself. Lost time injury as of 7/27/09 due to surgery.

Employee jumped up on passenger side of the truck to clean the window, he missed the step, fell back and when he landed on the ground, he twisted his left knee.

Employee was installing boards on props to protect SCSR storage area when a rib rolled over and fell, striking him on his back knocking him to the mine floor. The coal rib also struck his feet. He worked the next two scheduled shifts but started missing on April 14. His first Dr. visit was April 15 when a left foot fracture was found after xrays were taken.

Employee was using a pressurized grease system on 2085 lube truck to get a small amount of grease to put on the o-ring on a hyd. filter while servicing a D10 dozer. The grease system injected grease through the gloves and into the right hand, 3rd joint of middle finger.
