Accident type, without injuries

A roof fall occurred in XC #21 between entry #1 and entry #2 of the 1st main north off the 2nd main west, unit #6 MMU-067-O, spad #12+64. the fall was from the edge of the XC in entry #1 and measured 35' toward entry #2. The fall was the width of the XC, 19' x 8'H. there was no equipment involved, no injuries, and no disruptions to travel or ventilation.

A roof fall occurred in the last open x-cut in #0 Entry in the G-Butt working section MMU 002-0. 6' fully grouted Super twists bolts were being used. 5 bolts out of two rows were pulled out of rock and approx. 6' of unsupported roof fell this was at the face of #0 entry.

An unplanned roof fall occurred in entry #7 entry in the intake at spad #8844. This is not the primary escape way. 8500' from the active face, no passage of persons was affected.

An unintentional roof fall occurred on the 2 North 2, Entry 7 thru 8 Crosscut 17. The roof fall measured approximately 70' in length, 6-18' in width, and 5' in thickness. A 103 (k) Order (8440972), was issued March 30, 2012, by MSHA Inspector.

A mine examiner found a roof fall during a weekly examination of the return air course 2 breaks inby SS 3003 in #8 entry. The fall was 60 feet long, 20 feet wide and 6 feet high. The fall did not ventilation or travel.

A non-injury roof fall occurred in the #3 entry, 1 break inby survey station 649 of the 001 MMU. Fall was approximately 20'W X 20'L X 6'H and will not be cleaned up and has been breakered off.

At 12 pm Dave Chambers found a roof fall in F mains return approx. 40 ft long , located 32-33 brk # 2 entry

At approx 5:55 PM an inundation of methane occurred at the tailgate of the longwall face and at the MPL's in the back of the panel, in the panel #20 longwall section as a result of a cave in the gob. The mine was evacuated and MSHA notified when methane concentrations reached 4.98% at the MPL's. Ventilation was not interrupted and the methane began to go down almost immediately.

Experienced a fire in the Step 2 Agglomerator EER, Line 4 Main Kiln Drive. Fire was reported to MSHA at 3:35 a.m. and lasted 53 minutes.

Carbon monoxide samples in the recently opened West Lease, North/South entries began to demonstrate increasing and fluctuating values to a point that we determined to call MSHA. Sufco had been consulting with the price filed office concerning the sample results. There was not a sudden influx or inundation of gases.

An unplanned roof fall occurred 3/28/12. There were no injuries, blocked escape ways or eq. damage due to fall. The fall dimensions are 85' L. X 7' H. X 18' W. The fall area has been dangered off and all approaches to the fall have been cribbed and timbered. Loc. of fall Old #5 zone, 2blks inby spad #8097 # 8 entry 41-42 blk.

At the 3:38pm the spice lick elevator was taken out of service due to an electrical issue.

On the afternoon of Wednesday March 28th while drilling a twenty foot test hole at the mouth of unit 15 we struck a gas bleeder. Gas readings were taken and found to be in excess of .25% CH4. State of Ohio and MSHA were immediately notified at this point. MSHA inspector and Tech Services were dispatched to help evaluate the event.

At approx. 3:05p.m. a roof fall above the anchorage was discovered in the old #4 room of the 4 left off 4 North section return side. The fall was 17' W. X 300' L. X 8' H. The area was dangered off on all sides. Travel and air flow are not affected. No injuries or property damage.

A roof fall occurred in # 3 entry, 8xc of J Mains and meas. 25' in L. 40' W X 7' H. The fall was found on a preshift exam. Two pre-shifters were U.G. when cave was found. Entrances to cave were timbered and dangered off. Ventilation was not interrupted. Men were instructed on alt. escape way. Roof was bolted with 4' fully grouted resin bolts and 12' cable bolts.

this accident was immediately reported by phone. miner went into his work area and noticed the rib had sloughed and immediately came out of the mine and reported it to the mine superintendent.

A roof fall was identified at approx. 6:00 P.M. on the 3 Lt. Section the section was idle at this time. The fall was in the #5 entry at spad #9150 and measured approx. 70'L X 20'W X 6-7'H. The area was supported with 4' fully grouted glue bolts and 5' torque tension bolts. The area has been dangered off and cribs set. The are will not be cleaned up.

We were starting a cut in #7 heading and was cutting 10' on right side (curtain side) on blowing vent. When the right corner cut thru into a unknown old mine, there was some water every-one evacuated no one was hurt and no equipment was damaged no ch4 or bad air was in old mine.

Dozer operator smelled diesel fuel, shut engine off. When employee stepped out of the dozer, flames began coming out of the engine compartment. Employee discharged one fire extinguisher, the fire did not appear to diminish. Superintendent advised the employee to move away. Upon arrival the fire department could not reach the dozer. An excavator was used to put dirt on the fire.

A non-injury accident occurred on the secondary escapeway hoist. Hoist lost program and was not operating, mine was evacuated. Hoist program personnel arrived and reprogram and inspected hoist.

A Roof Fall occurred 21 crosscuts underground from the portal in the # 5 Entry. The fall measured 20'Long x 50' Wide falling from the gob hole thru #5 entry into #4 Right. The fall was approximately 4' to 8' thick.

An unintentional roof fall occurred on the 1 North 1, Entry 1 Crosscut 9 thru 10. The roof fall measured approximately 60' in length, 17' in width, and 6' in thickness.

During MSHA inspection while walking the southeast submain entry #1 we came across a fall. the size of the fall was 80 inches long, 25 inches wide and the height was 5 to 6 feet.

CSX (Railroad company) was working on the tracks using a cutting torch and a spark ignited dry foliage causing a brush fire. The fire ran onto our property and burnt estimated 15 to 20 acres. The fire was contained by the N.C. Forest service and local fire department, No employees were in danger nor was there any building damaged.

An unintentional roof fall occurred in the intersection of the no. 5 entry, 70' inby spad no. 97 on the 1 North Mains.

An unplanned roof fall occurred at the face of #1 Section #6 Entry. The roof fall was approximately 56ft x 16ft x 3ft 10in. The right continuous miner was involved and a clean up/retrieval plan posted. The continuous miner operator was struck by a piece of the rock and had a small abrasion.

The fall of ground occurred at approximately 12:30pm in the 3920 SFW travel way area. The ground fall was approximately 6' in width 5' in length and 3' in depth. The drift is approximately 15' wide by 15' tall. It was determined that some roof bolts in the area had corroded over time and gave way. There were no injuries, equipment damage, change in ventilation, or entrapment.

A roof fall was discovered by an examiner in the #2 entry of 6-Butt LT in the #2 Entry 800 feet outby the active 2-Southwest section. The fall measured 50-feet x 18-20 feet x 8-10 feet high. The roof was supported with 4-foot FG Resin Bolts on 4-Foot Centers.

Two contractor employees were riding the cement silos passenger elevator at the end of their shift. The elevator stopped due to an interlock switch problem. The two men were stuck in the elevator for approximately 1 hour until repairs were made. The elevator was equipped with a functional telephone.

Towards the end of the shift there was a fault in the auxiliary contactor on the north skip. This shut the hoist down creating an evacuation due to not having 2 escape ways to surface. The mine was evacuated with no injuries or other problems. An electrician corrected the issue and the hoist was back in service at 12:35 PM.

On 3/16/12, a roof fall was found in the #4 entry 1 bk outby spad #3122 on #1 Northwest Mains. Dimensions were 8' H. X 18' W. X 45' L. MSHA was notified by hotline. A ""J"" order was issued later modified to a ""K"" order to allow area to have additional roof support installed. This is not an active producing mine. No injuries reported, no EQ. involved No inter. of vent occurred.

Upon receiving a modification of a K-Order that was issued on D-Cell, we began filling D-Cell with water and tailings from the process plants. After we began discharging into D-Cell, the recently repaired area of the impoundment began to leak again. We immediately took action by discharging the water and tailings elsewhere.

Two men got in the K2 elevator on the sixth floor. The elevator stopped between the sixth and fifth floor. The radio inside the elevator was used to call the control room operator. A utility man was sent to the elevator and he pushed the elevator buttons which released the elevator. Elecutive Elevator Co. was called out to check the elevator.

An unintentional roof fall occurred on Unit 3 in Entry #4 at 66+00. The fall is approximately 8-9 ft. high, 17-18 ft. wide, 50 ft. long. The fall will not be cleaned up. The fall area has a slick sided slip running through the intersection.

The hoistman had been hoisting very sticky material which caused him to pump the hoist brakes to dislodge it from the skip. The over use of the brakes caused an unbalanced condition in the hydraulic accumulator which in turn caused a safety mechanism in the hoist to start setting the brake resulting in a hot smell. Because of this odor we shut the hoist down until it was checked

A roof fall was discovered in the 4WA Panel #2 entry between xc-7 & 8 and spad #4+48 & 5+12 in the #9 seam. The approximate size of the fall is length-15ft,width-18ft and height-6ft.

We had a roof fall measuring 4 to 5 feet thick X 19 to 20 feet wide X 70' in distance. This fall is a continuance of a fall reported on 1/5/12 on the 2nd North West Mains @ Spad #1341 in the #4 and #5 entries. This fall does not impede ventilation or travel-The fall will not be cleaned up-There were no injuries.

A non-injury roof fall was found by mine examiner. The fall was located 1cc inby spad #6654 on the 1-left bleeder panel. ""J"" order was issued later modified to a ""K"" order to allow additional roof support to be installed. Area has been dangered off. This fall will not be cleaned up. This is a non-producing mine that is in the process of being shut down. Fall was 40'Lx18""Wx8'H.

MSHA Citation #7636987 issued 3/23/2012 states a coal outburst occurred in 4N Longwall headgate that caused rib debris along travelway in entry No 1 along stageloader and in 17 x-cut between 1 and 2 entries. Report filed under protest and to be contested with Citation # 7636987.

An unintentional roof fall occurred on the 1 North Mains, in the intersection, 50'outby spad no. 134. The fall was approximately 20'w x 20'l.

A roof fall above anchorage was discovered in the Mains Intake Air Course at Survey Station 266 approximately 6000 feet from the Mains Section. The roof fall measured approximately 20' wide by 20' long by 5' high. All approaches have been dangered off and cribbed.

At 2:15 pm the #3 hoist went down due to a broken wear plate. During a test trip, the hoist operator brought the skip into overtravel and a wear plate caught on a guide splice, tearing it off. At 2:30 pm the official notice to evacuate the mine was given and MSHA was notified. The wear plate was replaced, a test trip was performed, and the #3 hoist was fully operational by 4:30 pm.

A fall of ground occurred at 4430-10R06 heading. There was an 8 foot round shot on 3-11-12 night shift. Ground support was not installed and fall condition was discovered by shot crete crew on 3-13-12. There were no injuries, equipment damage, change in ventilation, or blocked access as a result.

A roof fall was found in our P2-2 abandoned panel at x-cut 17, intake room 6-7. the fall did not affect ventilation, impede passage of miners and no one was injured due to this fall.

A condition was observed that the compensating chains on the No. 8 elevator were contacting water in the bottom pit when the car was in motion. The elevator car was removed from service until the water was pumped from the elevator pit. The No. 8 elevator was returned to service at 7:24 pm.

During the preshift exam the fireboss identified a roof fall on the 2nd Lt. Section 450 outby the face in the #5 entry. The fall measured 20'W X 100'L X10'H.The area was supported with 5'resin bolts and the intersection with 8' cable bolts. The area was dangered off and cribs set. The fall will not be cleaned up and the section will be moved.

An unplanned fall of the mine roof occurred on the 15 Left longwall tailgate. The fall measured approximately 4' high and was not above the anchorage zone of the installed roof support however it impeded the passage of personnel. Ventilation was not affected.

The A Shaft man/supply elevator was out of service for a period of time exceeding 30 minutes due to a broken fitting on a hydraulic line. No employees were entrapped as a result of the incident and the elevator sustained no damage.

On 3/9/2012 at approximately 6:05 pm a roof fall was discovered in an in-active area of the old 2 East Mains. This area is not required to be traveled. The fall was breakered off on all approaches. The fall did not effect travel of ventilation.Continuation 4/27/12 fall extends to Entry#7 and now meas. approx. 20' W. X 130' L. x 6' thick. Has been breakered off on all approaches.

A unplanned roof fall occurred 1 break inby of spad #13404 on the #3 section in #4 entry. The fall was 15' wide 16' in length. The fall was investigated by mine Foreman and MSHA inspector . The section has been stopped and turned left-handed 400 feet back.

A roof fall occurred in the Main North in #10 entry at #71 crosscut. The fall was approximately 8' high, 20' long, and 17' wide. The fall didn't affect ventilation, travel, damage equipment or cause any injury.

A roof fall occurred in E#3 @ station 30+10 in the Old 5A intake. The roof fall did not affect ventilation, cause personal injury or affect passage. The roof fall measured 27'x 31' x 6'. The area had been supported with 6' roof bolts. This report is being filed to abate a citation, we do not feel that it is reportable. Area had been dangered and timbered out prior to the fall.

The portal passenger elevator went down at approximately 10:15 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The elevator was returned to service at approximately 3:20 PM. A defective door switch, that allows the elevator doors to shut, was found to be the problem.

B-32 blade cat 16 h caught on fire while it was parked on the readyline. This happened between shifts. Extensive fire damage to the cab and engine compartment. No personnel were injured in this event.

A roof fall was discovered at 3X2 Crosscut, #21 Block (150' outby the feeder). The area was bolted with 6' SuperTwist bolts. No people, equipment, or ventilation was affected.

A roof fall was discovered during the pre-shift exam on 03/04/12. The fall is located near spad 1071. The fall measured 50 feet x 18 feet x 8 feet in thickness and was laminated shell roof. No escapeways or aircourses are blocked.

On 03/04/2012, an unplanned roof fall occurred on the 1A travel road, 1st SE Submain off the 2nd SW Submain at cross cut #8 in #5 entry. The fall measured approximately 18'W X 18'L X 9'H. The area was bolted with 8'and 10' point archer bolts, with additional truss bolts. The fall will not be cleaned up.

A roof fall occurred at 11:54pm on 3/2/2012. The inby part of the fan canopy plus 1 row of bolts fell. It measured 5-6' in thickness x 18 to 20' wide x 4 to 7' long. The fan was manually turned off.

A roof fall was discovered in the #4 entry in intersection. This area was mined in 12/31/2007. No one was injured, there was no equipment involved, and no airways were blocked. The fall measured 25' long x 20' wide x 8' high.

Roof fall between belt and track at Brk. 29 on 1 south belt. Fall is approximately 19 foot wide. There is a permanent stopping on belt side of fall and the intersection is cribbed on both sides.

A roof fall has occurred in the 3rd main east x-cut 133(103+20) in the neutral pillar line between entries #4-#5. The fall measured approximately 35'x19.5'x8' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension rebar roof bolts and was mined in February 2009. There were no injuries and ventilation and the passage of men were not affected.

The hoist car at Roadfork #51 mine malfunctioned for a period of time that exceeded 30 minutes. The battery voltage dropped below the normal limit causing the electric brakes to engage. No injuries were reported.

On 3/1/2012, an unplanned roof fall occurred on the 2nd Southwest Submain at cross cut #63 in #6 entry. The fall measured approximately 18'W X 18'L X 10'H. The area was bolted with 8' and 10' point anchor roof bolts. The fall will not be cleaned up.

A non-injury roof fall occurred at XC 19, #4 entry Northwest Mains, the fall was 18' wide X 30' long and up to 7.5' high. Primary Support 6' resin roof bolts.

A roof fall measuring 80'long, 20'wide, and 9'high was discovered in the 3 right entry off #6 belt mains inby survey spad 3026. The fall occurred due to laminated rock and coal above the 48"" resin roof bolts. No injuries occurred as a result of the fall and equipment or ventilation was affected.

Contact with electrical current

Electrician was performing work on a shuttle car cable reel on the West Main Left section when his received a fatal electrical shock. The electrician came in contact with the exposed 995 volt energized cable leads in the cable reel compartment.

Struck by falling object

This accident has already been investigated by MSHA. The victim was found approximately 250 feet inside the mine. He was working alone and had been struck by a fall of rib.

Caught in, under or between a moving and a stationary object

Employee was directing an end loader towards himself to hook a strap to it for removal of a dolly that was used to move the hi-wall miner launch. Contributing factors were attempting to hook a 6' strap while standing in front of this moving machine and stationary dolly. Cause was human error/bad judgement by the victim and end loader opr.

Struck by falling object

There were no witnesses to the event. Victim was found near a rib that had rolled and was not responsive. When found, crew members responded & performed CPR & transported the injured to the surface. The rib portion near the victim was approximately 32"" wide - 82"" long - and 0-8"" in thickness.

Employee was installing a canopy on the shuttle car and the canopy fell on him striking him in the head and chest.

The driller was assisting his helper to lower a tube. Driller slipped and stuck his hand out to catch himself. He caught the ring finger on his left hand. It cut just a little less than a quarter inch of the finger off.

While changing steel on #3154 driltech drill employee closed door on his left thumb causing an amputation below finger nail. Employee was taken to Brookwood Medical Center for treatment

The man was carrying a wooden crib block on his shoulder and walking down the entry. The man slipped on the wet bottom and fell down with his fingers over the end of the crib block. When he fell, his small finger on his right hand was pinched between the crib block and the mine floor.

Negligence to turn loader off to work on the air vent. EE dislodged the fan in the unit and it came on and he didnt have enough time to get hand out of way.

Employee was lifting or pulling and felt a strain in his abdomen. The employee was diagnosis with a hernia on 3/20/2012. His 1st day off was 3/20/2012

Miner was shoveling muck from the # 3 shaft pit when he felt pain and burning in his abdomen.

Caught in, under or between running or meshing objects

The employee was working alone to clear the kiln 1 multiclone rotary valve.The employee exposed the bottom of the rotary by removing the venturi. The employee ran the rotary to clear out pieces of debris. It is unknown why, but the employee placed himself near the running rotary valve. His hand was caught in the valve which severed his thumb, index, and middle fingers.

Over-exertion in lifting objects

Employee was lifting or pulling and felt a strain in his abdomen. The employee was diagnosis with a hernia on 3/20/12. His 1st day off was 3/20/12

Employee was moving a beam into place when the beam slid, pinching the left index finger between the beam and the concrete.

Employee was struck by a piece of falling coal. The employee's hand was caught between the piece of coal and a tray on his bolting machine. The piece of falling coal amputated the first joint of the employee's left little finger.

Employee was using cutting torch to cut metal jacks. He laid torches down to turn pipe and torch rolled over and flame struck left hand.

Employee had a cable tied to a mantrip pulling it during a section move. The cable became caught on a pallet of rock dust and caused the cable to become tight and striking him on the upper left arm and ribs. Employee worked until 4-5-12 when he saw the Dr. and was taken off work.

Employee was standing on a ladder using a sledge beating a pin out of a hoe bucket when the pin came out and knocked the ladder out from under him. He fell on his side hurting his ribs.

As Employee was walking from 27 to 28 seal he placed his left foot on edge of a hole and twisted his foot causing his ankle to roll. He was transported by EMS to CAMC General Division where he was diagnosed with multiple fractures of the ankle and had surgery to repair the damage.

EE tore muscle in right shoulder when arm was jerked from arm rest while trying to dig a rock out of a mine pit during a scraper operation. EE has missed no work previously but now has surgery scheduled for July 5, 2012 due to recurring pain. No work will be missed prior to surgery.

Handling material in the 9A section. Material fell and struck individual in right knee causing pain. Results of MRI were received on 4/12/12. MRI showed a dislocated patella.

While employee was on one knee bolting, a piece of coal/rock fell striking his ankle, causing the injury. The employee stated that he examined his work area prior to the start of his task, and he deemed the area safe for work.

Employee was roof bolting, he was pushing the bolt up with the head, the bolt bent causing the plate on the bolt to strike his left hand. He went to the ER and received five stitches in the palm of left hand.

EE was performing a normal pre shift inspection on the cone crusher. He checked the oil and stepped up onto the belt guard to inspect the cone. When he attempted to dismount the guard his foot slipped, he was unable to hold his weight with his hands and he fell on to the floor and handrail, bumping his head either on top/bottom rail. He was diagnosed with concussion.

Stated: Employee was installing a 6' bolt. He had part of the bolt in the roof and was bending the bottom half back to finish the installation. IEE was exiting the crusher/hopper. He fell 11.5 feet to the ground on his right side. Co-workers called 911 and 25 minutes later he was gernied by the EMTs then the helicopter showed up and was mediflighted to Amarillo hospital.

Employee slipped on the wet concrete, falling and bruising their right forearm. Employee went to doctor for ex-rays, there was no fracture, however doctor sent employee home.

The injured was walking next to the #38 Roof Bolter preparing to spot bolts on a corner. A piece of roof rock measuring 48"" X 19"" X 9"" thick fell from the mine roof causing the injured to fall to the ground. He suffered a compound fracture to his left middle finger. The incident occurred on MMU 001-0, XC #152, between #7 and #8 entries.

Employee was bolting top in #4 entry when a piece of draw rock fell striking him in the left shoulder. The rock measured approximately 3' X 2' X 1"" to 10"".

Pin was being removing from a pusher block on the highwall miner launch using a pry bar. He squatted down under the launch, pulled on the pry bar and felt discomfort in his groin. Cause was poor judgement in the means of removal of the pin. Victim & co-worker were cleaning a belt line w/ a jackhammer & vacuum truck. Oversized material plugged the 6"" vacuum hose, victim grabbed and removed the oversize material with his left hand. Breaking the material free from the hose end, his left arm was sucked into the hose up to his shoulder. Co-worker immediately activated a valve to stop the vacuum freeing the victim.

While kneeling down working on the miner, a piece of the coal rib rolled out striking him on the back and right shoulder. He continued to work full time without restrictions until the doctor put him off so they can do therapy and injections. ***He started missing work on 07/09/2012***

After completing installation a 480V circuit from the main 1000KVAJ4940/480V PA approved substation approval #MM - S82-11. The junction mounting block inside the 480V cable trough exploded when the last of the 3 25amp 25,000V fused disconnects were engaged.

Employee had surgery on 4/6/12.

While traveling along belt line on a mantrip in very low roof conditions, employees knees caught the roof and rolled him backwards. Did not miss work until May 29th, 2012 after doctors visit. Not serious injury told to rest lower back area for 1-2 weeks. Should be able to return to full capacity after that time.

On 04/30/2012, Injured Superintendent contacted me that he needs a MRI on Right Knee and he needs to report injury. I contested but Workmans comp will cover injury. Injured states he stepped on rock causing discomfort to knee. He didnt immediately report it thinking pain would stop. Orthopedic Dr. says he needs surgery for meniscus tear. At this time he has not lost any time.

Employee, was leaning over to pick up a water hose out of a highlift bucket when he slipped and fell into the highlift bucket dislocating his right shoulder. EE was transported to General Hospital by Pres. of Ali Co., where he was treated and released.

The injured was maneuvering a belly pan to install it on a dozer when he slipped on a piece of ice. He fell backwards landing on his left shoulder and injuring his lower back.

On 3/20/2012, employee was throwing a loop of miner cable out of roadway. He felt a pull and burning in his groin area. The employee continued to work his regular job. On 3/30/2012, our company was notified that surgery was required to repair the hernia. Surgery was performed on 4/2/2012 and the hernia was repaired.

Contractor EE moving overburden from one area of pit to another was descending the SW ramp empty when lost control of the Terex TA35. The truck drove into the W.berm/boulders which redirected him to the E.berm where truck drove up and thru berm resulting in truck upset & landing on the level below. EE was seen at hospital and released with minor injuries.

While waiting on the continuous mining machine to be repaired, the injured was struck by a falling rock. The rock fell from the mine roof which measured approximately 8.5 feet high. EE was grinding in the shop and he was wearing safety glasses and a welding helmet and he felt something go into his eye. He used Visine and thought it came out. A couple days later he told his supervisor he thought it was still in there and reported it to the safety manager. He then went to the eye doctor and medical doctor and had the material removed.

The emp. was evaluated @ ER and no med. treatment was administered and the injury was limited to bruising only. On 5/17/12 the emp. returned to the Dr. and a walking cast was applied. We feel this now meets the definition of med. treatment, therefore we are submitting this report as required.

4 man crew was rewinding a belt after prepping ends. 1 man was operating the winder while two helpers insured the belt wound onto the roll uniformly. Injured worker spotted a rock on the belt that he thought would damage the belt when wound onto the roll. He stepped onto the belt to kick the rock, his foot was caught pulling his leg in and fracturing his fibula.

EE was changing the bags out in the dust collection system. One of the bag and frame got caught in the main frame; he jerked the bag and frame. As a consequence, his finger nail is almost completely off. Employee was sent to the care center.

Fall onto or against objects

Employee was walking down a short decline about 10% grade when he stepped on loose gravel and slipped landing on his left side on top of his self rescuer that was attached to his belt. RTW on restricted duty.

Caught in, under or between a moving and a stationary object

The employee was moving a slusher. The Slusher caught on the wood deck floor causing the slusher to raise up approximately 12"" on one side. He released the slusher brake which allowed the slusher cable drum to free-spool out cable, which then allowed the Slusher to fall down onto his left foot.

Struck by falling object

Employee helping move kiln rig when a brick fell off of pallet and landed on his right hand. Fracture to middle and index fingers.
