Our Employee was advised by his doctor to take medication as prescribed, he neglected to follow his advice and passed out while at work. No bodily injury or damage/accident took place.

The hoist brake engaged due to losing air pressure through the air valve weep hole. Miners started evacuating the mine at 8:30 a.m. and were on the surface at 8:45 a.m. The hoist brake valve was repaired. The hoist was tested and miners returned underground at 11:00 a.m.

At approximately 8:15 am on April 30, 2012 an unintentional roof fall was discovered in the South Entries off the Main West (Unit 1)in entry #7 from SS# 1350 to SS# 1420. The fall measured 50' long x 19' wide x 8' high. The fall did not effect passage of men, disrupt ventilation, affect equipment nor cause injury to persons. The fall will NOT be cleaned up.

The hoist car servicing the #1 track slope became inoperable due to a malfunction in the switches discovered during the 60 day test.

A grass fire was noticed at 1:25pm at the north end of the D4 Mining area near county road 272. The local fire department and the state forestry department were called. The fire was extinguished at 2:30pm. No equipment or injuries occurred. MSHA was called at 1:35pm. CAUSE - spark or cigarette from a vehicle on the public county road.

An unplanned roof fall occurred at the Imperial Mine on 4/26/12 on the #2 section in the face of #5 entry intersection and through the crosscut 5-6 at spad #8803. There were no injuries or equipment damage due to the fall. The fall will be cleaned up according to the company fall clean up and securing plan. Imperial will also follow guidelines put in place by MSHA & the WVOMHS&T.

Power outage occurred causing the fan to go off which led to the employees being withdrawn from the mine. Employees were evacuated from the mine via the primary escapeway elevator.

A roof fall occurred in the Intersections of entry #3, entry #4, and cross cut 19+96 in the 1st Main East off the Main North. The fall is 9,400' from the face. There were no injuries, no equipment involved and no disruption to ventilation or travel. It is undetermined if the cross cut between the entries is involved.

A rock fall was found in 7th North Panel off the 2nd East Submain in #8 entry at 1+00 in intersection. It measured 8' Wide x 16' long x 5' high. This area was mined approx. 12/14/2011. The area has been timbered around.

The date and time of fall is unknown it was located by the weekley examiner during his travel of that air way. It will not be cleaned up and does not effect Air Flow.

Roof fall was discovered on 4/22/2012 two breaks inby SS#1553 in #5 entry. Fall was approximately 7' high by 20' wide by 60' long. Fell from #5 entry over to #6 entry. This area was developed with 6' fully grouted roof bolts on about 5/8/2011. Fall is 2800' outby working section.

Employee was getting off 345BL Excavator when he noticed smoke coming from under the engine compartment. Employee shut off engine and grabbed fire extinguisher and attempted to extinguish fire but was unsuccessful. He stepped back and called for help. Excavator burned until help arrived.

A roof fall measuring 140' long, 19' wide, and 8' high was discovered in old works # 1 entry inby survey station 3265, 9 right off 8 Mains. The fall occurred due to laminated rock and coal above the 4' resin roof bolts. Ventilation, the passage of men, and machinery movement was not affected. No injuries occurred as a result of the fall.

During the daily examination of the hoist at Camp Creek a bolt was found broke off on the brake caliper on the drum. A new bolt was installed and the hoist was put back in service at 1:09pm.

An unplanned roof fall occurred at X-cut #13 in the #4 entry in the intersection on the new way track. The area was bolted with 6 foot torque tension bolts. The fall was about 20'W X 20'L X 6' High. The fall will not be cleaned up and it has been cribbed and dangered off.

An unplanned roof fall occurred in #3 entry about 1200' from #3 unit, across from spad # 58833. The area was bolted with 6' torque tension. The fall was cribbed and has been dangered off. The fall was about 18' W X 19' L X 8' high.

An unintentional roof fall was found in the intersection of the No.# entry at c.c. 16. The roof fall is est. to be 30' W. X 20' L. X 6' H. There was no property damage or injuries. The fall may be an extension of an existing fall that was found in the xcut to the left of the intersection.

At approx. 9:25 PM a roof fall above the anchorage was found in the E entry of the 3 right off 4 left off 4 North section. The fall was about 30ft long X 18ft wide and 6-7 ft high. The area was dangered off. There were no injuries or property damage. Fell at 8+15.

An unintentional roof fall occurred on 001 section during pillaring operations. The fall was approx. 8' H. X 20' W. X 25' L. It was approx. 10' inby survey spad #6317 in the #6 entry. It fell between 2 Fletcher M.R.S. trapping the 2 M.R.S.'s MSHA hot line was notified. All EQ. in the affected area was moved to a different location. No injuries were reported. All EQ. recovered.

At approx. 1:00a.m. an area of shotcrete fell from a rib at CC75 between room 24 and 25. The area is approx. 25'x50'. No EE's were present when fall occurred. The affected area was barricaded to prevent entry and an outside contractor was called to determine corrective measures.

An unintentional roof fall occurred on SM#2, #6 entry, break14. The fall consisted of shale rock and was 50'Lx19'Wx9'H. The area will be bolted with 8' fully grouted torque tension bolts.

Area of fall was in old works used for a sump area.The area was just outby another fall we had 12/29/2010. Fall was approx. 5' high 16'-18' wide and 20'-25'long. It was approx. 10'-15' inby the #7 Entry, East Panel of the 1st So. Panel. Seasonal change/humidity probably a factor. No damage or injury.

Operator was pushing spoil on Cut 5 in the C pit when he noticed smoke coming out of the engine comp. He then seen flames and exited the machine. EE attempted to retrieve the fire extinguisher at which time the fire was to hot for him to do so. All agencies and the Fire Dept. were called.

The #1 elevator was out of service and no one was on the elevator. It was determined by Schindler elevator service that the brake needed cleaned and adjusted. The car was put back in service at 7:40 pm.

Early into 2nd shift the hoist (Koepe) tripped out due to low air in the compressor. This shut the hoist down creating an evacuation due to not having two escapeways to surface. The mine was evacuated with no injuries or other problems. The issue was corrected and the hoist was put back into service at 6:20 am on 4/17/2012.

Examiner discovered fall in the #3 entry at break 201. The fall was approximately 20' wide and 30 ' long.

A fall of the roof occurred on the 8th NE Mains x-cut #8, #2 entry intake primary escapeway (10A) 20' length by 15' width by 4' height.

Workers were removing bolts to repair a crack in the housing of shacker below a bearing. After cutting a bolt with oxygen/acetylene torch the hot bolt fell inside the shacker box and down into the discharge chute. The hot material caught in the chute liner and after 30 to 35 minutes caused the chute liner to burn. The fire burned approximately 30 min.

Unplanned roof fall occurred at bottom of slope #4 entry at spad #206 track entry. The fall was approximately 30 ft. long x 18-19 ft. wide x 15 ft. high. No injuries or entrapment or equipment involved. No ventilation involved. Secondary escapeway blocked and it was changed around the fall.

An unplanned roof fall occurred in the primary escape way 1 cross cut outby the fan shaft in the #8 entry. No personal injuries, no equipment was involved. Passage of persons was affected.

An unintentional roof fall occurred on the Main West 2, between the Beltline and Intake Stopping Crosscut 99. The roof fall measured approximately 30' in length, 18' in width, and 9' in thickness. A 103 (k) Order (8433781), was issued April 15, 2012, by MSHA Inspector.

Roof fall was in the old works in #5 and 5-6. Spad # 57+50 The fall was 18'x 60'long in #5 entry and wrapped around 5-6. The fall was reported to MSHA at 12:05P.M.

Two employees were trapped in an elevator at our course crusher at NSM Babbitt mine site. The employees were never in danger, the elevator had gone into emergency stop.

The 5 North Emergency Hoist would not start. All work was stopped in affected area. Battery was discharged. Corrective Action: ""Changed Battery"" Elevator released at 10:45a.m. All agencies were notified.

During the weekly air way exam the examiner encountered this roof fall in the number 7 entry South West Mains 2. This fall is located in a worked out area and did not effect the air way it will not be cleaned up.

The operator is filing this 7000-1 under protest and to comply with the mandatory abatement provisions of the act. Constructing a pond, personnel filled the pond to ensure impermeability. The next morning the water level had lowered. MSHA was notified immediately. No soil disturbance or material failure was discovered.

While performing system checks on the automatic hoist at Shaft #1, a programming glitch occurred, making the hoist inoperable. The programming glitch was identified and corrected, allowing the hoist to be put back into immediate operation after being inoperable for approximately 50 minutes.

An unplanned roof fall occurred, discovered at 10:20 a.m in the 2nd Southwest mains at the #79 crosscut, between the #9 and #10 Entries.

Hydraulic brake system for the production hoist was overheating and was unable to be reset. The problem was evaluated and correcting by replacing hydraulic brake system control valve.

A fall was discovered in the B Sub-Mains Section (#46 Block, #4 Heading). The location was outby B-5 Panel. The area was bolted with 6' SuperTwist bolts and 12' CC-Cable Bolts. No equipment, ventilation, or people were affected by the fall.

The #1 hoist pinion brake sensors became out of alignment. The mine was evacuated at 11:00 pm and completed at 11:10 pm. The pinion brakes sensor mounting brackets were realigned, tightened and hoist was tested. Miners returned underground at 1:30 am.

A non-injury roof fall occurred at approx 8:50 AM. 2nd cross cut on Ginsong Mains. 18' wide x 5' thick x 38' long.

On 04/10/2012 approximately 7:00a.m. a fireboss identified a roof fall in the number 4 entry located at 9 break on 6 belt at SPAD number 3512.The roof fall was 20'Wx80'Lx5'to6'H.Area was supported with 4 foot fully grouted resin bolts and the original mining date is unknown. The roof fall will not be cleaned up and timbers have been set and the area has been dangered off.

An unintentional fall occurred at #20 crosscut on the Main North entries @ 14+50' between #5 & #6 entry. the fall was Approximately, 18' wide by 20' long by 10' high.

An inundation of methane and hydrogen sulfide gases occurred in the Mains section, in the #93 x-cut between entries #3& #4 while the miner was cutting in the face and encountered a fault/void. A CMI was on the section and immediately issued a 103K order an began his investigation.

A reportable roof fall was found in F Seam, entry 5, between crosscuts 25 and 26. The fall was approximately 20' long, 18' wide, and 6.5' high.

An unplanned roof fall has occurred on the #1 Section outby #4 spad 3426. The roof fall was approximately 20' x 20' x 4'. This area is 12,800ft from section and is inby stopping at EP#13B. There was no equipment involved and no injuries reported.

Towards the end of the shift the Hoist (Koepe) kicked out on high amps. This shut the hoist down creating an evacuation due to not having two escapeways to surface. A fall of the roof occurred on the 3rd S.E. Panel #2 Unit outby section between intake/return at cross cut #2, 100' length by 16' width by 5-8' height.

An unplanned roof fall has occurred on the #3 Section #4 Entry 1 break inby spad 13439. The roof fall was approximately 20' x 40' x 8'. No equipment was involved or no injuries were reported.

The #1 elevator was out of service and a person was on the elevator from 4:00 pm to 4:27 pm on 4/7/12. It was determined that the elevator door was not completely closed. Cleaning and inspection by Schindler elevator service was completed at 7:45 pm on 4/7/12.

An unintentional roof fall occurred in the 2nd West intake @ 42+13 station in the #4 entry. The roof fall did not affect ventilation, passage of men or cause personal injury. The fall measured 20'x 20' x 10' high and had been supported with 6 foot fully grouted roof bolts. The fall has been timbered off and will not be cleaned up.

The roof fall occurred 2 x-cut inby spad# 5172 which is located in F Mains. Area bolted with 6' fully grouted bolts, also 8' cable bolts. Fall was 21' H. X 45' L. X 25' W. Area was cribbed and W8x31 Impact Resistant Square Set was built and set threw the cave area.

The agglomerate discharge conveyor 2350 caught fire. The ignition source was a failed bearing on an impact idler. Confirmed fire at 4:49AM and extinguished fire at 5:33AM

EE was using the air compressor on his service truck to blow off the air filter of the front-end loader. He was standing at the side of the service truck when the air compressor, mounted inside the truck bed, failed and ruptured. There was damage to the back bed of the service truck and also to the rear of the cab. EE was not injured.

The portal passenger elevator went down at approximately 8:50AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The elevator was returned to service at approximately 10:30 AM. A defective door switch, that allows the elevator doors to shut, was found to be the problem.

A roof fall occurred on the #1 Section #4 entry break 15 while retreat mining. The area affected included the intersection to the push-out that had just been mined.

At approximately 8:15am on April 4, 2012 the Shielder elevator became inoperable. The solenoid on the brake mechanism failed and would not allow the brake to release. Parts were ordered and repairs began immediately. No one was trapped or injured. The elevator was back in full operation later in the day.

An intersection in S722 fell above the anchorage. Estimated measure at 40'x50'x4'high in exhaust entry in 1st intersection inby the barrier pillar in S722. Remainder of abandoned panel barricaded.

A 1 inch diameter compressed air line ruptured which is utilized by the man hoist. The line was repaired and the hoist returned to service 4:05pm, April 4, 2012.

An unintentional roof fall occurred on the Main West 1, Entry 2 thru 3 Crosscut 52. The roof fall measured approximately 50' in length, 17' in width, and 5.5' in thickness. A 103 (k) Order (8441324), was issued April 3, 2012, by MSHA Inspector.

An area in 2nd East Submin #4 entry in pillar line 40+40 to40+90 some rock had fell out between some bolts and loosen up a couple bolts so the area was cribbed out to keep anyone from entering. When it was looked at on 04/10/2012 it had fell in some approx. 22'long,x 19'wide, x8'thick. On 04/03/2012 it was 16'long, x8'wide, x 4'high and was dangered out.

A roof fall meas. 5-8' X 20' X 60' occurred 100' inby spad #2429 in the #6 intake entry. This area is approx. 25 c.c. outby MMU 005-0. The fall will be dangered off and will not be cleaned up.

WHILE MAKING HIS WEEKLY EXAMINATION OF THE RETURNS THE EXAMINER CAME UP ON A FALL IN THE NORTH SUB-MAIN RETURN AT X-CUT 12 ENTRY # 1.

A flash flood occurred that dropped approximately 3"" of rain in a 45 minute period. A drain pipe stopped up allowing water to overflow the drainage ditch and allowing water to enter the belt portal of the mine. This did not affect ventilation or safe travel for personnel coming out of the mine.

Flint Ridge Mine #2 has experienced a non-injury unintentional roof fall approximately 2000 feet outby the 002 section in the return of 4 Main in #1 entry. The fall was approximately 8 feet thick, 20 feet long and 50 feet wide in an intersection. The area was initially supported with 60 inch fully grouted rods. Laminated shale gave way due to deterioration.

A roof fall approx. 60' x 50' x 20' high was discovered at the top end bleeder, 9C-Rt Panel off #2 North Mains.

The Koepe counterweight came out of the guides at the Koepe bottom area. The mine was evacuated and repairs were made and test were completed. The Koepe was put back in service on 4/3/2012 at 6:00 am. There were no injuries and no other issues.

A non-injury roof fall occurred in the Northwest Mains at break 33 of old #6 belt, 1 x-cut inby spad 2044 in the #1 entry. The fall was approximately 25'L X 19'W X 7'H in an intersection.

Roof fall was found on 4 West Mains Cut Out. Fall is located 20ft. inby SS#3171 in entry. Fall measured 40 ft. X 20 ft. X 8 ft. Fall did not go into intersection.

A Caterpillar 321 excavator caught fire and burned. Accident was called in and inspected. Inspector determined that a hydraulic line burst causing the fire.

A roof fall has occurred in the 2nd subpanel southeast between x-cut 36-37 in the pillar line return neutral. The fall measured 50'x6'x18' and was caused by a kidney bed in the mine roof. The area was supported by 5' fully grouted roof bolts and was mined in March 2012. There were no injuries and ventilation and the passage of men were not affected.

Fall from scaffolds, walkways, platforms

Worker took it on his self to cut belt line down without the use of the equipment that we was using to take it down with. He cut it then started walking away from it as it was falling. Then it caught the catwalk he was on and flipped him over the double hand rail on the catwalk and fell to the ground about 20 ft down.

Fall to the walkway or working surface

Miners were removing a counterweight from an excavator, standing side-by-side, when the counter-weight dropped unexpectedly to the ground.The victim stumbled backwards,fell landed on the back of his head on the shop floor. He was unresponsive during CPR. The autopsy report concludes the proximate cause of death as""commotio cerebri""noting also cardiac arrest.

Over-exertion in lifting objects

EE was in his knees shoveling a belt drive. When he tried to lift a shovel full of material his knee slipped causing him to strain awkwardly which resulted in a suspected hernia.

Caught in, under or between running or meshing objects

Roller mill operator was finishing the routine cleaning of #1 roller mill dust collector system...was standing on the deck beside the collector chute, reached inside access door and down the chute with right hand feeling for material... made contact with operating roll feeder...result is partial amputation of two fingers and laceration to a third

Caught in, under or between a moving and a stationary object

The employee pinched the end of the left index finger between a piece of plywood and the wall onto the plywood was being fastened.

Caught in, under or between a moving and a stationary object

Employee was working on a exploration drill rig. He was steadying the lower end of a piece of drill casing as it was being threaded to casing that was already installed in drill hole. As it was being lifted into place, the employees right hand got mashed between two pieces of casing. He sustained a laceration, fracture and partial amputation to his right pinky finger.

Over-exertion in lifting objects

Employee was installing a top belt idler and pulled a muscle in his stomach. On 5/23/2012 employee was diagnosed with a hernia.

Struck by falling object

Employee was helping mechanics put a pin in a 992 boom when a pin above fell and crushed his pinky finger.

Caught in, under or between a moving and a stationary object

Employee was installing center bolts with a roof bolter. He asked other bolter operator to back up a little. Other operator said something, he turned to ask what and placed his hand in the scissors of the ATRS. The other operator lowered the ATRS crushing 3 fingers on his right hand.

Over-exertion (Not Elsewhere Classified)

EE and the crew went to bank and stepped off of the boat and felt a slight discomfort in his foot. He stated that it was nothing, but by Wednesday May 2nd his foot was stiff and hurt. He requested medical treatment and was taken to the clinic. He reported it on May 2, 2012. EE is our weekend watchmen on the dredge.

EE stated he was assisting fellow EE's advance the 16 L section conveyor belt. While doing so the section feeder became hung up in mud on the mine floor and the EE's were attempting to pull it with a shuttle car. EE was pulling the excess SC cable into a crosscut to protect it from damage when SC operator started SC which activated the cable to move, struck legs and FX ankle

Was moving miner cable by himself, lost footing and felt something pop in lower back and twisted right knee. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Employee was bolting in the face of #1 entry on #1 unit when he was putting up his rib bolt and rotated the bolt against the roof and the rotation hose blew and struck him in his left eye.

Struck against stationary object

He was unloading bags of rock dust from a supply car on the 11 A miner Section. He was placing a bag onto the can duster when he stepped on a rock and turned his left ankle.

Over-exertion in lifting objects

EE was lifting a 3"" pipe to place it in a pipe stand. He felt a pop in both the lower and upper arm. Felt faint after injury and was treated by a first medical responder taken for more medical attention.

Fall from ladders

Contractor was working approximately three feet off of the ground, standing on a four foot step ladder. Contractor shifted weight causing him to lose balance and fall to the ground. Ligament was torn in right knee.

Employee was instructed by the foreman to operate the roof bolter and the CM this shift when he refused. The work was re-assigned to the employee when he complied. Before he started operating the roof bolter, employee states that he stepped off the bolter and stepped into a hole where he twisted his knee. There was no witnesses to this incident and it remains under investigation.

The miner placed his right thumb in a pinch point between the tilt cylinder and idler on a CAT front end loader.

Resulted in a fracture to the second metatarsal on the right foot. X-Ray could not be confirmed until 4/30/2012 due to inflammation. Employee worked on modified job duty until 5/7/2012 then went out as LTA due to Corporate policy.

As the employee was applying the pipe wrench, the shaft turned which pinched the employee's right ring finger between the wrench and fan frame. The finger was broke in 2 places. Surgery was on 4/6/12.

Employee was using a pressure washer to clean the outside of a scrubber prior to welding repairs. His face shield became fogged, and he accidentally struck a scrubber support with the nozzle, causing it to change direction and strike his left leg with hot water. Employee inside gyp silo 21 breaking down the job. Standing on scaffolding--was handed a ladder from below. Went to turn with ladder. As he did his foot caught between scaffold planks. They were tied down and did not move. He fell sideways wrenching his back on the lower left side.

Operator noticed smoke/flames coming from his haul truck. He activated the fire suppression system and attempted to leave the truck. There were some flames near as he exited the cab of the truck so he attempted to remove the fire extinguisher but changed his mind and exited the truck without the extinguisher. He got some burns on his arms and a small spot on the side of his face.

Injured employee was holding an extension ladder for another employee.(rung latches were not fully engaged) When employee put his weight on the ladder, the ladder started to collapse, pinching injured employees right thumb between the first section and the fly section of the ladder. Employee suffered laceration to right thumb, received stitches, and placed on restricted duty.

On April 19th employee stepped down off a rock, his step was approx 1-2 feet down off the rock. He came to work on the 20th but was complaining how his leg was bothering him, he went to the Doctors office on the 20th his doctor said he tore his femoris muscle and did not want him to work the rest of the day.

Left middle finger was lacerated when it was caught between the two halves of an elevator door when the injured was closing the door. The straps installed to enable pulling the door closed without exposure the pinch point were not used.

Absorption of radiations, caustics, toxic and noxious substances

Employee was wrapping up a grout line and cement entered his right eye.

Using a 7"" Buffer on a stainless Vessel when the buffer jerked forward pulling his right elbow.

Fall from machine

Employee changed oil and fuel filter on Komatsu dozer and was descending dozer when he mis-stepped and fell backwards to the ground landing on his rear and back.

EE was reaching into tool box when lid fell striking his right hand, lacerating his knuckle.

Employee reported back pain with no incident. Stated he had pain on 4-18-12, does not know how reported on 5-3-12 when he presented with a personal Dr excuse for work. Awaiting MRI results to make determination of occupational illness.

Over-exertion in lifting objects

The employee was attempting to move a rock that weighed approximately 40 lbs to use it for a fulcrum to extricate rock bolts that were buried in mud. He was not in a good position to lift the rock and it caused him to rotate his arm while lifting and this is what caused the injury.

Fall to the walkway or working surface

EE was working in his area of responsibility when he heard a loud noise and which startled him. He fell back landing on his hands where he got abrasions on both hands. EE placed on restricted duty.

After hosing down floor in crusher building, employee slipped and fell to floor. Injured knee and back. Returned to work light duty.

Employee inserted hand into air conditioner vent contacting fan with middle finger on right hand. Resulting in an 1 1/4"" laceration requiring two stitches.

Miner was climbing down a ladder way with a metal lid covering it. As he was closing the lid he lost his grip in the lid and it fell on his left hand that was resting on the edge of the railing. The lid caught the thumb between the lid and the railing.

EE was jarred while operating haul truck. EE notified supervisor, but initially refused any medical treatment. EE worked her last shift on 4/18/2012 was off two days and came back for her first scheduled work shift on 4/21/12 and requested to be taken in to clinic. EE was placed on restrictions.

He was walking won steps at the Prep Plant when he slipped and caught himself with his right arm causing pain in his shoulder (right). *We dispute the allegations in Citation #8055881 and submit this report solely to terminate the citation.

Employee was hammering shaft to remove bearing. Sliver of metal broke off shaft and struck employee on the wrist. Sliver embedded 1/8 inch into skin. Sliver was removed by a doctor.

Employee stopped to observe the bolter install an anchor bolt for a shuttle car, believing that the bolter was already in position he started to walk past the operators deck just as the bolter operator moved the bolter closer to the rib pinning employee's leg against the rib causing a laceration and requiring stitches.

Employee 1 was assisting Employee 2 with the removal of core from a core sample tube. Employee 1 was holding the tube in an upright position, while Employee 2 was tapping on the tube with a dead-blow hammer (bowling pin). Employee 2 struck Employee 1's thumb in the process, resulting in a laceration to Employee 1's right thumb, requiring seven stitches.

Employee was removing pound-down skirting with a 4 lb hammer. He hit the trougher frame with the hammer and his second finger, left hand was between the trougher frame and hammer handle.
