At 10:17 am on 5/31/2012 the mine encountered a failure to operations to our elevator. After troubleshooting the elevator, it was found to have a blown control fuse. The fuse was replaced and the elevator was put back in service at 1:00 pm on 5/31/2012.
A roof fall was found off the tailgate entry of the (22nd West) of the 23rd West longwall. The roof fall was approximately 16 feet from the tailgate and was approximately thirty feet in length, twenty feet in width and approximately five feet in height. The roof fall block access off the tailgate.
A non injury roof fall occurred at this mine, the roof fall occurred on the #3 belt at break 10 in the crosscut between #5 and #6 entry crushing out the stopping separating the belt from the primary intake. 40' by 6' high.
An unintentional fall occurred at the #82 x/cut, on the 2nd west travelway. The fall was 20' X 20' X 10'.
A fall of roof occurred in the 2nd Right panel off the 3rd Southwest Submain at crosscut # 47, # 6 entry. The fall measured 25 feet long x 30 feet wide x 12 feet thick. The area was supported with 5 foot fully grouted roof bolts.
A roof fall occurred in the #5 entry spad #2+80 off the 1st south sub-main off the Sturgis main on the #2 section. The fall measured 18' wide by 25' long by 10' high.
While mining from the tail towards the head, an alleged ignition occurred while cutting bottom on the tail drum of the 19 A longwall face.
An unplanned roof fall was discovered in-between survey station 1533 and 1515 in the intake air course that travels toward the number 2 and 3 section. The fall was approximately 60' long, and 16' wide and 4' high. The area was dangered and breakered off, as well as plotted on the mine map.
A roof fall occurred in the 2nd East Submain in #5 entry at 40+90 intersection. It measured 45ft. long x 18ft. wide x 8ft. thick. The fall was loaded out cribbed around and rebolted.
An unplanned roof fall occurred one x cut inby spad # 59453 in between #7 and 8 heading Outby the 35 section. The fall was about 25' long,18' wide , and 7-8 feet high. The fall will not be cleaned up.
A roof fall was discovered in the 5B tailgate in the crosscut at 27 wall at spad #27+80 Dimensions were 15' high x 15' wide x 20' long. No injuries. Did not affect passage or ventilation.
Towards the end of 2nd Shift the North Skip got hung up in the scrolls damaging the hoist. This shut down creating an evacuation due to not having two escape ways to surface. The mine was evacuated with not injuries or other problems. The hoist was replace and returned to service by the next scheduled shift at 10:20 pm on 5/29/2012.
At 4:00 am the slope hoist was removed from service due to an electrical issue in the hoist panel that occurred after a power outage. The hoist remained out of service until repairs could be made.
Roof fall at x-cut 111 on the Headgate #1 unit between the number 2 and #3 entry in the x-cut. It did not block the ventilation or the travel past it. The fall measured about 18 feet wide by 20 feet long, and fell out about 8 to 9 feet high. We set more standing support under and around this area. In the form of cans.
Roof Fall on #3 Entry of 2-Southwest Section. Roof supported with 6-foot FG Resin Bolts and various lengths of cable bolts (10' - 12' - 16'). Ventilation was not disrupted. Slicken-sided roof was noted in the stack rock produced during the fall.
While operating the Hitachi EX 1100-3 shovel loading haul trucks with stripping material, the operator noticed flames on the equipment. The employee exited the cab immediately and Eagle Crushed Stone's emergency action plan was initiated. MSHA and the local fire department were notified. The Hitachi EX 1100-3 shovel sustained significant damage.
A roof fall occurred above the anchorage at 17-09 c.c. between the belt and track. No injuries. Not an escape way, travel and ventilation have been affected. Both sides of fall have been cribbed off. The fall is 40' in length x 15' high x 15' wide.
Discovered a roof fall at the top left panel on the intersection at 1st south east submain in left return airway entry #2, Spad # 0744. The fall was 4400 feet from the face. The fall was 30 feet long by 20 feet wide, and 4 to 6 feet in height.
At 7:15 pm the Spice Lick elevator was taken out of service due to mechanical issues.
An unplanned roof fall occurred at X-cut #27 on the new way track. The fall was about 20 feet long, 18 foot wide and 5 to 6 foot high. The fall will be cleaned up and rebolted.
During the weekly Hoist inspection the shaft crew found a spring on the south skip that needed replaced. The mine was evacuated to perform the work and the crew went ahead an replaced the whole south skip. The skip was put back into service on 5/23/2012 at 6:20am. There were no injuries or other issues.
Roof fall occurred on the 5B Section in the Tailgate Entry of the working section at road location 2+60. The fall was approximately 20 feet long and 8 feet high.
Continuous miner cut into unknown water well.
An unplanned roof fall occurred in the Roadfork #51 mine, 1 brk. outby spad #303, that measured approximately 60' long X 20' wide x 5-6' high. No injuries were reported. The area was developed on 6/5/08.
Slope hoist car was down due to dead batteries.
Stress from displacement from 22' fault, not enough cribbing in the intersection. Lost 3 joins of 4"" water line.
A planned detonation in the slot barn produced a fire ball; which in turn resulted in a grass fire. The grass fire was extinguished within 15 minutes. Due to the nature of this event and its unknowns, MSHA was notified.
The employee was positioning the boom truck to lift a pump. While he was lowering the boom the wire rope cable came in contact with the 25,000 volt power line. The employee was not injured however the boom truck received damage to four of its ten tires.
An empty train traveling from Winchester, VA. to Gore, VA struck an OTR Delivery Dump Truck at the Quarry Grade Crossing. Dump Truck Driver stated he did not see or hear the train until he was on the tracks. (Driver was listening to I-Pod with earphones in ear)
AT APPROX. 2:20 AM A ROOF FALL WAS FOUND IN THE NORTH SUB-MAIN TRAVELWAY X-CUT 23.
At approx. 11:35 PM a roof fall above the anchorage occurred in the main west rooms @ the 2+50 of the #3 room intersection. The fall was approx 17' X 20' and 6 ft high. The area was dangered off. There were no injuries or property damage.
3:35pm it was determined ground had fallen above the anchorage zone of bolts located at A64O xc. 3:48pm the condition was reported to MSHA.A squared up round was shot consisting of a right rib slab and floor. Previous inspections of the area did not show reason to install additional ground support beyond our normal bolting plan.
The hoist operator noticed smoke coming from the AC wound rotor motor so he immediately shut down the hoist. Upon examination it was apparent that the rotor had made contact with the stator windings. Since this rotor and stator were not a matched set it is suspected that there are some tolerance issues. We still had two operational hoists to facilitate access.
While making his airway the examiner identified a roof fall on the old 2nd Left Section in the #5 entry near Spad #11025. The Fall was approximately 20'Wx100'Lx5'-6'H. The area was supported by four foot fully grouted resin bolts and the original mining date is approximately 12/07. The roof fall will not be cleaned up and the area has been dangered off and timbers have been set.
There was a malfunction in a regulator at the Avista Electric Company substation which supplies power to our facility. This malfunction resulted in a high voltage situation which tripped our high voltage relays causing power to kick out. There was no one in the conveyance when the power tripped. As a precautionary measure we evacuated all underground personnel.
Elevator over traveled causing manual reset of the elevator.
There were two locations on the clean coal stockpile that had flame showing. One was on the south side of the west end of the pile and the other was on the east end of the pile. There were no injuries.
Employees were bolting according to our plan, when they heard the roof start to work. They went to ask there supervisor for guidance, and upon their return to the bolter, the roof had fallen onto the bolter. The Safety Manager was called at 0709 that morning (14 MAY)
Pulled slope car out of the mine and found loose wraps on wheel. After trying to un-spool the cable they found that the cable was damaged. New cable and termination was done.
A roof fall was discovered during the second shift pre-shift exam at survey station 196 and continues one break outby. The roof in this area was shale with sandstone streaks and was bolted with 4 foot resin bolts and supplemented with 10 foot cable bolts. The fall measures 90 feet long x entry width x 7 feet in thickness. Timbers were installed to prevent access to the area.
IGNITION: Occurred on 8 A belt at the tail piece, in the tail piece frame. Employee was burning holes in the header frame, while burning the holes, sparks went in between the bottom of the header frame causing a ignition. The flame lasted 10 seconds or less and was put out by a hand full of rock dust.
We experienced a rock burst in the 168 stope on 4600 level which shattered rock on the footwall rib approximately 2' to 4' deep for a distance of about 16' in length. Rock also appeared to have been pushed up from the sill creating a muckpile approximately 3' to 4'high. There was no apparent damage to the back except some surface cracking of the cemented backfill. No crew present.
A Roof Fall occurred on 3 Left Panel at SS# 1209 to SS# 1227 in the #1 return entry. The fall measured Length 105' x Width 18' x Height 7'.
The mine had a roof fall at survey station #10 in an intersection. The fall measured 30' L. X 7' Thick X 12' W. An addendum to the approved roof control plan will be submitted to MSHA to rehab the fall area.
An unintentional roof fall occurred in the No. 1 entry (return) 5,300 feet outby the section. The fall measured 30' long, 20' wide and 4.5' high. The airway and travel of men was not affected.
A spoil side slide occurred in the B-1 Pit. There was coal upheaved and buckled in the pit floor thus a call to MSHA was made.
MG shut down on high north flywheel bearing temp. Hoistman attempted to restart set by turning up the thermostat. Second trip was @ over 200dg. F. A/C motor was making noise (rotor rubbing stator. Hoistman shut down and called (Night shift Foremans) and #3 and Coeur Hoist.
A roof fall occurred in the intersection of entry #6 and xc 29 in the 1st Mains North off 2nd West off Main North. The fall is 50 x-cuts outby the face. There were no injuries, no equipment involved and no disruption to ventilation. The fall was in the travel road but travel was easily routed to entry #7. The fall will be cleaned up.
A rock fall was discovered at 11 break in the #2 and #3 entry (1 break outby 12A seal) the fall was estimated to be 75' long X 20' wide X 4' thick the area had been supported with 4' fully grouted bolts. no injuries, ventilation was not interrupted and no equipment was involved. the fall will be timbered and dangered off. the fall will not be cleaned up.
An unintentional roof fall occurred on East Mains #2 entry, break 4. The fall consisted on shale rock and was 60'L x 19'W x 10'H. The area was bolted with 8' fully grouted torque tension bolts.
A rock fall was found in 1st South Panel off 1st East Submain in #9 entry at 12+00 to 13+00 in intersection. It measured 100ft. long x 19ft. wide x 8ft. thick. The area has been timbered and cribbed around.
An unplanned roof fall occurred along the #6 belt in the #4 entry at spad #3105 6500' from the active face.
The 003 section was pillaring in the #5 entry when the roof set down on the #2 JH Fletcher Mobile Roof Support (MRS) causing it to get stuck. We spot bolted with 8ft. bolts from the outby intersection to in front of the MRS, and set timbers before removal.
Operator of a CAT 980G loader had just taken a bucket of coal from the stockpile, when the loader quit. He turned to check the problem and found a fire under the hood. He proceeded to fight the fire with other mine personnel helping. The local fire department was contacted and arrived on scene to finish fighting. No one was injured
Kiln 5 experienced a ignition of coal dust that was contained within the mill system. No injuries or damage resulted from event. MSHA hotline was contacted immediately.
A fireboss conducting a weekly examination found a roof fall in the 5th west section between #46 and #47 x-cut # 4 entry. The roof fall measured 30'L x 16'W x 7'H. while investigating the fall a second roof fall was found # 5 entry # 48 x-cut 20'L X 18"" W X 7' H it was determined later that the second fall may connect with a old fall at # 47 x-cut in the # 5 entry.
At approximately 11:45am, a roof fall was identified on the 3 left section ( 3 ) breaks inby 8c Head in the #4 neutral entry. The fall was approximately,20'W x 20'L X 5'-6'H. The fall was supported with 5' torque tension bolts. The fall will not be cleaned up and has been dangered off and timbers set.
Tracking system was down due to electrical problems.
An ignition occurred on the E-3 section at 2:07 am. The power was de-energized and all government agencies were notified.
WHILE MAKING THE WEEKLY EXAM A FALL WAS DISCOVERED IN THE MAIN WEST RETURN AT X-CUT 3 ENTRY # 2.
A roof fall was discovered in the 3-Butt, #2 entry, #4 crosscut during the pre shift examination. The fall area was approximately 40-ft long, 16-ft wide and from 4 to 6 ft above anchorage. Fall consisted of laminated and stack rock. The fall was immediately called in and reported to both MSHA and the State.
At 11:57 a.m., the slope hoist began having issues with the electrical system and was taken out of service and the mine was evacuated.
unintentional roof fall inby spad #1622 - does not effect travelway or ventilation
During retreat mining on the 001 MMU, and mining the last lift in #6 pillar, draw rock fell on the head of the CM. This pushed the head of the miner into the gob, to the extent that the miner would not tram on its own. No injuries resulted.
An unintentional roof fall was discovered in the # 6 Entry near spad #952. The fall measures approximately 70'L x 18'W x 6'H. No injuries resulted from this occurrence.
A partially loaded cast blast pattern detonated unexpectedly when an adjacent cast blast pattern was fired. The two patterns were separated by a shot break consisting of two rows of empty drill holes. The unplanned detonation caused damage to a small equipment trailer and broke several windows on an unmanned overburden drill.
Employee was walking near Gypsum Storage Building to return to the Finish Mill Building. Employee may have been struck by a piece of mobile equipment that was working in the area.
Employee was operating an excavator between two small ponds when the ground beneath the excavator tracks failed and the excavator toppled into one of the ponds.
An employee suffered fatal injuries after he fell down the hoist-well from the 3rd floor onto the basement floor of the preparation plant. The employee was on a ladder when it slipped or shifted causing the employee and ladder to fall over the handrails and down the hoist-well. Employee was transported to the ER by EMS.
Still under investigation. ** The miner was operating a loaded 40 ton articulated haul truck down a slope when the truck went out of control and hit a rib. The cab section overturned. The victim was ejected from the truck. **
Employee was helping to pour concrete in the shaft, the hose is directed to the shaft walls as needed. The hose was over loaded with material when the concrete came out of the bucket too fast, this caused the hose to surge and knock employee and his co-workers off their feet, resulting a fracture to employee's left ankle.
Hernia caused by over exertion or strain
Employee was operating miner, checking his sites before cutting when he was struck by a hauler amputating his leg. Co-workers applied tourniquet, treated for shock and cared for him while traveling out of the mine to the helicopter where he was flown to U of L for treatment.
Injured and coworker were replacing a pump. there was not enough clearance for the pump to slide over the pump well hand rail. The new pump became caught on the hand rail. Injured lifted up the pump. The pump slid across the hand rail catching injured' s left thumb between the pump base and hand rail resulting in a severe laceration. Hospital personnel amputated the thumb.
Employee was attempting to dislodge rock from crusher when he pinched finger with pry-bar. Broke middle finger of left hand above 1st knuckle. Tip of finger was removed by doctor at emergency room.
While in the process of removing the two piece drill steel from mine roof, the top piece separated from the bottom piece. Bottom piece fell striking injured employee on his pinky finger resulting in a amputation to the tip of finger.
Employee had stayed over after 2nd shift to bolt up. The 3rd shift lead man called the bolter operator over to show him where a roof bolt needed to be spotted. As the lead man walked away he heard a sound and when he turned around, the bolter operator had been struck by a piece of draw slate. The slate caused non-displaced fractures to his pelvis and lumber facets.
He was stacking a skid of stone, when he lifted wrong and pulled something in his lower back.
Employee was working on top of the crane boom trying to pin the equalizer, using a sleever bar to move the equalizer, when the equalizer slipped back, striking the employee causing him to fall between the lacings of the boom. His harness caught him before he hit the ground, but he struck and caught his left leg on one of the lacings causing a fracture to his lower left leg.
He was working at the end of a guillotine when lifting a rock he felt a pain in his lower abdomen.
Employee was loading boards into scoop when a rock fell striking his neck and shoulder. Employee went to Good Samaritan sustaining an abrasion/contusion and was ordered to remain off work until the 4th of April.
Employee stepped off back of truck and came down wrong and twisted his ankle. He was taken to hospital where it was x-rayed and it showed he had a sprained ankle. No permanent damage.
The injured sprained his right knee when he slipped while pulling a piece of ventilation brattice cloth out of mud. The employee worked until 6/14/2012 when this injury become a reportable injury. Further medical examinations revealed a tear in his right knee that has to be surgically repaired. Lost time began on (6/14/2012).
EE was walking behind the bolt machine when he tripped over a rock and twisted his knee.
While changing out of his rain suit and rubber boots, the injured was putting on his normal work boots when he heard a loud pop and his foot started hurting. He fractured his foot while putting on his boots.
Employee was helping move MRS, he was 40' back watching the cable and the MRS operator was 25' away from MRS when he pressurized it against the mine roof. The bolt end broke off and became like a projectile striking employee on the lenses of his left eye glasses breaking the left lenses and striking the left eye.
Core Drill Helper (Victim) was pulling a tube out of the rods when he strained his back. He was brought to the E.R., told to rest over the weekend. He saw an occupational medicine phys. on 6/5/12 and release to work.
Employee had changed a pressure gauge on the continuous mining machine, employee was positioned at the rear bumper of the machine, he asked the machine operator to start the machine, the machine operator bowed the head of the machine causing the machine to go to the mine floor mashing the employee's foot.
After drilling first drill steel he put in second one. When bringing the drill box down along with the steels, the top one fell out of the bottom steel and the room. The drill steel hit the right eye.
Injured employee was installing tensar on the overcasts for 17 Left on the 1 South Mains section. While spading the screen into place a section of rib rolled loose and struck the employee's left foot resulting in a fracture to the metatarsal bone.
Employee was conducting the Pre-Op on a truck when he stepped on a piece of material causing him to fall and injure his left leg. He was seen by the ER physician and excused from work.
He was stacking stone when he reached for a small stone and heard an internal pop in his lower back.
Employee was performing maintenance when hot metal contacted the torch hose causing it to ignite. While trying to crimp off the hose the employees right hand was burned when it came in contact with the flame. The burn was treated and the employee will regain full use of his hand.
Employee stated he was cleaning under the belt when a jack became dislodged and struck him on the back of the head.
Employee was coming onto shift, could barely get off bus & walk up the stairs -- he went to medic on site & stated that he has had low back pain for awhile. There was no incident, no accident, no injury. He had back issues with his previous employer. He was seen by doctor on 5/30/12 & Dr. issued restrictions in which we could not accomodate.
While watching shuttle car cable slack get reeled up, ready to signal when to stop, the cable flipped over got behind employee grabbed his feet and back and slammed him face first into the rib.
Employee was stepping off tractor & rolled his ankle. Went to medic on site. We took him to ER - x-rays were negative, however, restrictions were issued in which we could not accommodate. This is a LT injury.
Miner was injured performing maintenance on a conveyor belt bearing. Sustained an eye injury when foreign debris entered his right eye.
Employee was helping arrange soap tanks so that they could be unloaded with a forklift. As he was moving the tote, he felt a sharp pain in his lower back. The employee continued to work until 6/19/12. At this time, the employee was taken off work and remains off work until test can be conducted.
Employee slipped in the mud along the belt line and twisted his left knee. He continued to work until he had surgery on 6/22/12.
Employee was lifting a bottom roller when he began to experience lower abdominal pain. He was seen by the ER physician and diagnosed with a Abdominal Hematoma and excused from work.
Employee was dismounting LHD Number 11 when he slipped because of a dusty hand rail and steps. Employee struck head and back when he slipped. Employee refused medical treatment at first. After, he decided to go to the doctor because of the bruises. Doctor placed employee off work on June 11, 2012.
Pulled a piece of stone on roller conveyor into splitter and strained his back.
Climbing down from front end loader. Missed last step. Lost his grip and fell backwards.
Employee was shoveling coal on a screen. He felt a tingling in his right shoulder and down his right arm.
Picking rock at feeder and rock hit hand and caught hand between rock & mine roof.
Employee was struck by a roof bolter cable that knocked him into a coal rib. He received abrasions to his left shoulder and a laceration to the left side of his face. He received 5 sutures to close the laceration on his face.
Employee was operating a John Deere 850J dozer clearing brush when the dozer traveled onto a stump and fell off, causing him to strike his left shoulder against the inside of the dozer cab. The first day of work missed was 7/13/2012.
Employee was conducting the Pre-Op check on the truck when the dirt/ground gave away causing him to fall and injure his right knee. He was seen by the ER physician and excused from work.
EE fell from a Caterpillar 777-D Rock Truck. It appears that the employee was transferring his dinner bucket from one rock truck to another from the operator's deck to the next rock truck off side fender that was 4ft. away. He over extended himself and slipped and fell between the two parked trucks to the ground. EE has 3 broke ribs and brusing on his left side.
Ee was working with the drill steel on a roof bolter when he pulled a muscle in his back. He is in physical therapy. Should be back to work in three or four weeks.
Employee was cleaning belt drive and take-up, he was shoveling gob into a wheel barrel. He was pushing the wheel barrel to dump it when he strained his left elbow. Employee was seen at the Er.
Operator was working on fuel filter on dozer. Stepped back and fell hitting left knee, was transported to Summersville Regional Hospital by ambulance, referred to a specialist for ongoing treatment
Employee was working with the Roller Mill pneumatic crane chains. He tried to drive a stretch of chains up by hands to unravel the base of the crane, and during the action he injured his back. He was complaining of back pain.
Employee was carrying a block of wood and slipped while walking on leach pad, he attempted to catch himself resulting in an awkward fall injuring his Rt. Ankle.
While riding in the personnel van from the prep plant to the bath house, the driver of the van applied to brakes and employee fell forward and hit his face on the dashboard and windshield. This caused a laceration under his left eye. First aid was applied and he started to travel home. On the way home he stopped at the ER and was told he had a hair line fracture in his cheek bone.
Walking to move miner cable was walking down small incline, slipped and fell landing on his back. Bottom was wet and muddy at time of incident.
He was tailing a guillotine with two men standing at the end splitting stone. A piece of stone from one of the two men broke off and hit him in the abdomen.
A coal rib rolled out striking employee's lower leg and foot causing a hair line fracture above the ankle.
He was at Plant III helping the welder, he had to pass the belt of the crusher to the stairs he lost balance and fell on the belt hitting it with the right side of his back, causing a bruise.
Employee was getting off the loader, he was on the bottom step and lost his balance causing him to slip and fall.
EE was sorting stone in the pit with a loader, EE stopped and exited the loader, when stepping off loader EE stepped on a rock and rolled foot over causing a broken bone on the top of the arch of right foot.
While lifting a bag with 2 sets of come alongs in it the injured felt his should pop. It appeared to be out of socket.
The injured was stepping out of the deck of a roof bolter, it was on top of a rut. When he stepped out of the bolter, it slid over against his foot/ankle. He did not start missing work until 5/29/2012 due to this condition.
Employee was stepping off a tractor when he felt his left knee pop. Employee did not start missing time from the injury until 08/20/2012.
Employee was bolting teeth onto a loader bucket. When he picked up the teeth, he felt a pain his groin area.
Employee was doing a coal yard inspection. As he was walking back to the Loader, he twisted his right ankle.
Employee's duties consist of adding drill rod to casing. Two men lifting 120 lbs. On this day he said his back was hurting.
Throwing #2 chute switch on slope bottom when switch handle came back, jerking his left shoulder. Employee experienced pain and missed 6 days work over the next ten months. No issue with PPE, compliance, mining machinery, or training.
Near the end of his shift on 5/24/12, Employee stepped in uneven mine floor in an outby area and hurt his left ankle and foot, and his right knee. Employee did not fully report incident until 5/29/12. Employee did not begin missing work due to the injury until 06/06/2012.
Employee was scraping grease off the bottom of a cone head and cut his L H pinky finger on a sharp edge of the sealing.
Top was hanging about 50' behind trailer and 50' or more behind line shields. Top bumped and broke and fell taking out supports on crib line striking employee in his ribs and head. The top took out 3 complete cribs and approximately 60 crib blocks. Blew crib block from 5' to 9' from where they were built.
Operator of left bolt machine was cleaning out dumper on dust box on off side when the bolter helper dropped the slab jack causing the flapper on the dumper to close smashing his finger.
Miner dropped an electric drill on his hand Thursday 5/24/2012. Results of his injury was a break to his hand. He did not tell his supervisor or seek treatment until he returned to work on 5/25/2012. Office was notified on 5/29/2012 that he will be off work until the hand is healed.
Employee kicking in drill holes reported twisting knee. Off site doctor conducted exam, no injury but follow up testing scheduled. Follow up results of exam on July 3, 2012 showed a tear in employees right knee. Unknown at time of report if any further action to be taken.
While roof bolting and at the completion of installing a 5' bolt with a pizza plate an area of top coal adjacent to the rib fell and struck him on the head. X-rays revealed no fractures or other obvious injuries. He was restricted from returning to work for 2 days while his condition was evaluated.
EE was welding on #2 elevator when he started to weld got a shock.
While backing the bolter out of #9 Entry, the bolter began to sink in the bottoms and slide towards the rib. After hitting a big rock, the bolter jumped, throwing the operators foot out of the cab and smashing it between the rib and the bolter. He received a fracture in his left foot. ***He began missing on 06/04/2012.***
EE was repairing parking brake on haul truck. His hand was on drive shaft when operator accidentally put truck in gear. EE's hand was pulled into drive shaft which jerked his entire right arm in a circular motion. EE refused first aid and drove himself to local ER He was treated and released.
Ee was riding the mantrip out at the end of the shift. His foot got caught on a piece of belt and pulled him so hard that he fell from the mantrip and broke his ankle. Surgery will be required and he will be off work for several weeks.
EE sprained his lower back while assisting in the rescue and CPR efforts of an employee immediately reportable accident on 5/23/12.EE reported the incident to Penni Carpenter the evening of June 7, 2012.
Miner was preparing to move tracking system reader. When he cut the wire to the reader, the reader swung down and struck him in the face/mouth - striking his teeth. Employee did not miss work until June 8 thru 13, 2012.
He was tailing a guillotine when he rolled a rock off the pile onto his left foot.
Employee was getting out of skidsteer loader, caught right foot on headframe and fell down on right shoulder. Shoulder strain.
Employee was traveling in a mantrip and as he reached up to open the D-rail switch at #32, he injured his right arm.
Employee was holding pin punch by handle while second employee used sledge hammer to strike pin to remove master pin from spare excavator track. Employee felt foreign object break skin and enter left forearm when pin punch was struck.
EE was walking along 18W belt at the booster drive when stepping on a rock and twisted the right knee. EE cont. to work until 7/9/12 when he was removed from work to have surgery to the right knee.
Drive shaft broke because driver shifted on hill, truck rolled back down hill and overturned. Brakes were working - driver panicked - let truck roll back down hill. Driver was not wearing seatbelt.
Employee was jacking on a Low Boy, that had become de-railed at #148 break, when the jack rolled out striking the employee, and injured the employee's right foot.
Stepped out of truck onto a rock wrong way and felt a pain.
Three employees were walking on a decked walkway when the injured employee stepped and the edge of a below deck access plate pivoted allowing the left leg to fall through to the hip level. Revised to DAFW when surgery was done on 6-11-13, no restricted duty, medical treatment or no days away prior to 6-11-13.
EE was shoveling coal from around belt structure & lost his balance & fell against belt structure - 30"" coal seam - 2 other employees were there at that time & asked him if he was ok. He replied he was ok & continued work. Asked him if he wanted to go to dr & he said No - however he changed his mind & went. He was off for 2 days. Has a history of missing 1 to 2 days per week
On May 22, 2012 an employee was assigned duties as a trainee in the weld shop. He was air arcing on a bucket when the employee felt slag fall between the bucket and his right leg. The employee then noticed a hole was burning through his pant leg. He slapped his leg to put out the fire and in doing so, the slag fell into his right boot resulting in a burn just above the right ankle.
Bates Contractor was bolting an overcast when a piece of drill steel came out of the hole striking him on his 3rd finger on his left hand resulting in a fracture.
He was hanging back miner cable, and water line when he felt pain in lower back area.
Operator slipped getting out of the excavator. He had a minor ankle sprain (left one). He was given first aid and taken to the hospital to verify his left foot.
Employee was positioned between two (2) parked golf carts, a third golf cart was attempting to move and stuck the side of one of the parked golf carts. His right knee was pinched between the two (2) parked golf carts. No surgery was required. He began missing time 5-24-2012.
He was banding a skid when he grabbed a rock and spun fast to his left and felt a pull and then burn in his left knee.
Employee was carrying a 20ft long piece of pipe and tripped over track rail and fell on back.
Employee had drilled his first steel up getting ready to drop to add his second steel, when a rock fell down the steel, striking his boom, breaking in half, and then striking him on the right leg above his knee but below his hip. Employee finished his shift. The next day it was stiff, resulted in a deep muscle bruise.
EE was stepping off the left side of a Caterpillar Dozer D9N as he stepped onto the push arm step twisting his ankle and fell to the ground, injuring his mid to lower back.
Individual was crawling when he noticed he was having pain in his right knee saying the pain was unbearable due to excessive crawling.

Employee was driving forklift. Fork caught in bridge deck. Sudden stop forced employees ribs into steering wheel.
Employee was helping to install a chute on a 6x20 screening plant. He was using a prybar to help align the chute when the chute shifted and his right ring finger got caught between the chute and the screening plant. His finger was fractured. He saw a doctor on 5/23/12 and was released to full duty. Following his doctor's appointment he went on his scheduled vacation.
On May 21, 2012 an employee was assigned duties as a laborer in C Shop. The employee was in the process of getting on a floor sweeper. His left leg was on the sweeper and his right leg was on the floor. When he started to get on the sweeper he heard a loud pop emanate from his right knee.
Wet, muddy Miner slipped on loose structure entangling right foot in structure.
Changing screens in the power screener - felt a burn in his arm when he picked up hammer to swing. Employee refused to go for medical treatment. He said it would be fine. On 6/4, he said his arm was still hurting, worse and decided to seek medical treatment after all.
Employee was pumping grease in miner conv. grease jack when grease fitting blew out, throwing grease into both eyes.
Employee was bolting w/jackleg, when a rock fell & pushed jackleg back, pinching finger between bolt & drill handle, resulted laceration. went to medic. Worked his next shift then went on days off. later, finger started to look like it was infected, went to ER. finger was cleaned out, x-ray showed a bone chip. Restrictions were issued, we can not accommodate. This is a LT injury
The area was wet and slick, employee fell and landed on his back.
Employee was cranking the handle on the water hose reel when he felt his right shoulder pop, causing pain. He was seen by the ER physician and excused from work.
Employee alleges he was using an angle grinder that he had modified and the wheel came apart causing lacerations to his right leg.
While walking around a parked shuttle car, the employee stepped on some loose material on the mine floor, causing him to shift his weight. He felt pain in his left knee which got progressively worse.
Employee was checking the MRS cable on the offside of the miner when a rib rolled striking his right lower leg.
The employee riding a man-trip to the outside accidentally extended his left arm beyond the seating compartment where is was caught between the man-trip and rib. The event resulted in cut/sutured injury.
Injured was moving miner from a completed cut in the #6 Entry to begin mining in the #9 Entry. The tail piece of the miner caught the injured employee and wedged him at the corner of the #6 Entry.
Removing drive roller from belt drive, roller fell striking a bed rail that put it in flight which struck employee in jaw cutting face skin and fractured jaw. PPE was not factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner knowledge was a factor.
Employee was lifting 2+20lbs buckets of bits. When he turned to stand upright he heard a popping sound in his lower back. Going to his knees he could not stand up or straighten his back after that.
Changing drill bit on F-9 drill, his hand touched the hot bit, burn to hand.
While changing a 637 Scraper Tire, the employee lost his grip on the tire flange that was positioned against the tire and on a 8"" X 10"" X 48"" long block. The tire flange drop onto the employee's left foot. The caregiver diagnosed a foot fracture.
While the employee was loosening a 4"" pipe cap with a 3' pipe wrench he felt pulling sensation in his back that caused a strain.
EE was working on track rail in the mud. His foot became stuck and he fell over striking his back.
Employee was working in Digestion Dept performing routine preventative maintenance. He was exposed to a plume of steam that contained caustic mist. He started coughing and walked to Medical to report.
Miner was operating a wheel loader in pit loading sand into hopper. After filling bucket at the bank and backing away he had double vision. Miner believes it occurred when the bucket contacted the bank.
Miner alleged he was climbing ladder on floating conveyor. Ladder measured about 5.5feet high. He allegedly lost footing and fell. Upon investigation miner does not remember any more information and this incident was not witnessed. Diagnosis was a concussion and lumbar contusion.
While bending over to pick up dragline power cable, the injured strained his back.
Employee was walking around scoop, stepped on something and twisted his right knee.
EE stated that he was assisting two other EE's to loosen a coupler between a 6"" and 4"" discharge line in the 1 Left Bleeder aircourse. A co-worker was using a ratchet to attempt to loosen a rusted bolt. While the EE was prying on the bolt, a rope broke that was holding the discharge line causing the injured EE's hand to be caught between the two coupler resulting in a bruise.
EE was getting on unit #1140 end dump. The first step was high and when he pulled himself up onto the ladder he felt something pull in his right arm. Later he could not lift his arm about halfway in the air. EE worked the 17 went to Dr on 18 and was not scheduled to work that day. Not scheduled on 19 and 20 went back to Dr on 21 and released to work with no restrictions on the 22.
Employee was working on the blast pattern when he slipped. As he fell he caught himself with the right arm causing pain to his right shoulder. Employee worked normal job. Went in for surgery 10/22/2012, became lost time.
Employee was prying with slate bar, slate bar slipped, causing employee to fall forward putting his body weight on his upper arm, employee did not report accident to magmt. until 5-21-12, employee was seen by a physcian for muscle strain.
The employee struck his calf of his right leg on a piece of metal, causing a blood vessel to burst in his calf muscle. This required two incisions to relieve the pressure to his lower leg.
Employee was walking when he tripped over a berm and cables falling to the ground. Employee was taken to Good Samaritan where they couldn't diagnose his condition of his ankle at the present time due to a pre-existing condition. Employee has to see specialist in Indianapolis and will remain off work for an uncertain period of time.
Employee was carrying a vulcanizer utility knife on belt when he tripped landing on knees. The knife was not fully closed which caused a small laceration above the right knee. Employee was treated at Raleigh General Hospital where he received three (3) sutures.
Employee was attempting to remove a rock that had fell on the track. As he was lifting on the rock, he felt a pain in his back. Employee continued to work up until 5/29/12 and was then taken off work.
Miner was walking around the wash plant area observing the operation of the plant, when he stepped on a ridge of gravel, causing him to twist his ankle, resulting in a fracture.
Pushing on a shot and backed over a big rock. Jarred the victim. Had pain running from right hip to right ankle. Was x-rayed and released.
While mucking with loader hit a pot hole and bounced in seat, when seat bottomed out felt pop in lower back. No missed work until 7/25/12
One employee was throwing cinder blocks through a mandoor to the victim. Through a lack of communication, victim started to pick up a block as another one was being thrown resulting in a pinched finger. Right ring finger required stitches and bandaging.
Operator was moving a horizontal lift drill to another location in pit area when an unsecured drill steel on machine fell striking a miner in the forehead (miner was wearing hard hat) and approximately 15 ft. from suspended load.
Employee was going to answer the phone and put his knee on pipe when it rolled and his knee popped and twisted.
Got out of scoop. He felt pain in lower back.
Miner was reinstalling guards on CV10 after repairing the pulling and an other miner pushed on the guard smashing his hand between the guards. (crusher was not running at the time).
Employee was shoveling the power head on the highwall miner, the miner was on a steep a grade and the beam he was standing on was wet and muddy. The employee slipped and when he tried to catch himself he pulled something in his back.
Two Employees were cleaning around a conveyor belt, one on the tail pulley, one on the head pulley. The employee on the tail pulley was leaning on his right hand, his hand slipped and his left arm got caught in the conveyor. The employee sustained a fracture to his left wrist.
Employee was struck in the head with falling debris and buildup while working in the pocket. He was evaluated on 5-15-12 and released back to work. He took himself off of work on 5-16-12 and 5-17-12: returning to duty on 5-18-12.
EMPLOYEE WAS STANDING ON A LADDER TIGHTENING BOLTS ON A PIECE OF EQUIPMENT WHEN THE WRENCH HE WAS USING SLIPPED OFF THE NUT, CAUSING THE EMPLOYEE TO LOSE HIS BALANCE AND FELL FROM THE LADDER
Dismounting from machine operator placed left hand on the step instead of handrail, his ring hung as he made contact to ground with his feet, ring cut into backside of ring finger.
Employee opened a tool cabinet when a hammer fell out and onto his left foot.
Employee was lifting two bags of rockdust from the scoop to make room for a pallet of dust when he felt pain in his low back.
EE was in 8 South moving materials in dinner hole when he felt a pop in his right elbow. Diagnosed with a torn right bicep tendon. Started missing 5/31/12.
Employee was attaching fliger type duster to scoop bucket when latch handle snapped back catching his small left finger between the handle and the side of scoop bucket, was seen at ER, six sutures and positive fracture of tip of small finger
Individual was talking on a mine phone at 19-W 2 block when a vehicle was coming through the track doors. Individual turned to get back in the cross cut when he fell over a bag of rock dust. Individual complained of back pain, pain in right shoulder and arm. Individual continued to work until 12-10-12, could no longer work
Employee bent down to pick up a box of caps. He stated he felt a sharp pain in his back and fell to his knees. After he set down for a few minutes he was able to return to work with no problems. The next day he felt pain in his back again and went to the doctor.
Employee stated that while exiting #36 jeep to throw the 14 left section switch when he caught his foot on the operator's seat causing him to fall to the mine floor resulting in a fractured left hand.
Employee was on top of a de-energized continuous miner. He stepped up on a flare board on the miner tail and slipped off striking his left leg on the top of the flare board. He received a laceration to his left shin that required 6 sutures to close.
Injured employee was assisting in removing the covers from the tail drive motor on the 15 Left longwall face. Employee caught his right middle finger between the covers and the frame of the motor when the cover was pushed by fellow employee resulting in a laceration requiring sutures to close. Employee was wearing glove at the time of the incident.
Injured employee stated that while making up a bolt to be installed a rock measuring 18"" wide X 20"" long X 4"" thick fell from the top and struck his right arm resulting in a fracture.
Employee pinched finger between the feed belt skirt and conveyor idler when trying to release the skirt.
Employee was wearing boots with holes in them and walked through water on the floor that contained caustic.
Walking by a trailer employee was started by a cat coming out of the weeds and tripped and fell on his leg breaking it.
Employee ruptured a tendon in his right shoulder while using a wrench to loosen a plug in the differential of a CAT 834 RTD.
The employee was operating a 777 cat trk on the haul rd when he stated he hit a bump in the rd. At which time he felt a pinch in his lower back and tingling. The employee had stated that he had previously had problems with his back when in the military in the 1990's
While shoveling spillage onto #6 conveyor belt, injured's shovel got caught between the belt and mine roof, jerking his body. This caused pain in his upper back and neck. He is off work seeking treatment.
Injured was pulling a piece of belt structure from the supply hole. The area was wet and muddy and the structure was tangled. His feet stuck in the mud while pulling and he felt pain in his back.
Employee was welding on a shuttle car when a piece of hot slag burned through ear plug and fell into ear canal causing damage to ear drum. Employee continued to work full duties up until 1/31/13 when he was scheduled off for surgery.
The employee had climbed onto a diesel pump to hook a chain to it, to lift the pump out of the area with an excavator. The employee slipped and fell onto the pump.
Employee was handling a 13.8 V cable (that was partially coiled), while picking up the coil- he walked over a berm and twisted the cable to align the pins in order to plug in the cable. He felt pain in his lower back while doing this task. He was evaluated by a Physician on 5-12-12 and taken off of work. He returned to duty on 5-17-12.
Miner was tightening bolts on a screen shaker with a wrench when the tool slipped off of a bolt causing his hand to strike the side of the shaker. Miner was taken to hospital on 5-11-12 but no fracture was found. He complained that his hand was still sore. Took him back to hospital on 6-12-12 and fracture was found.
Employee was turning a piece of stone on table preparing it for cutting when it spun too fast creating a laceration on his left finger
Employee was plugging a cable into a battery motor when the cable shorted out.
Employee slipped while installing a 16ft cable bolt and injured his left leg. He was diagnosed with a chip fracture on his left ankle.
Employee stated that the carrier he was on bumped another carrier which caused him to hit his head. This caused a laceration in the back of his head that required sutures.
Moving oil drums, injuring his lower back
Employee was roof bolting on MMU 004-0 in the #4 entry. While putting up his inside bolt a piece of rock fell measuring 39"" x 26"" x 6"" came inside the man-up striking employee in his left leg, causing tear to the Achilles Tendon.
Operating a shuttle car and slid into the corner rib and pinned his upper left arm between rib and shuttle car
Employee was pulling CM cable and felt pain in his right forearm. This did not become a lost time until 5/22/2012. At this time he was taken off work until he completed physical therapy.
While working on the T-Block taking marble off of block a piece of marble laid over on thumb an mashed thumb. Then while standing by header saw passed out an then fell an hit head on the floor.
Employee (roof bolter operator) was rolling up a wing curtain in the intersection and when he bent down on one knee, a piece of gob fell out and hit his back and left ankle. He began missing work the following week.
While using a cutting torch to remove a bearing the employee was burned by the torch tip after another employee tripped over the torch hose causing the torch flame to contact his left hand.
Employee said a timber fell across his back. This is being investigated.
Taking down some plates from the pickup the rope broke injuring the left foot.
The miner noticed a lump in his left groin area on May 10th 2012. He visited his family doctor on May 25th 2012 and it was determined to be a hernia. A specific incident or action that could have caused the hernia is unknown, nor is it known if the hernia was caused by activities related to the miner's tasks.
Employee was traveling on the road on the way in, when the man trip hit a bump and employee hit his head on the canopy. Employee thought he could work but came out before the end of shift wanting to go to the doctor. Employee sustained a neck strain/sprain and was ordered to remain off work for an uncertain period of time.
Employee states that he had back surgery before coming to this mine, he said while lifting belt rollers he felt like his back was beginning to hurt him. He said he was going to go to his doctor and have it checked out to see if his back fusion was acting up on him. His doctor was scheduling an appointment. Status unknown at this time.
Employee was rocking dusting by hand in the number 4 left break and as he was spreading dust his back started hurting.
While inside sag mill shoveling off rocks on the trommel screens, employee felt a lingering pain in lower back. Employee was bent over shoveling at the time of incident.
EE was standing beside the roof bolter machine making up 6' bolts when a piece of gob fell out hitting her on the back of her head causing her face to hit the bolter. Her nose began to bleed and when she got outside, she was taken to the ER to see if the nose was broken. It was not broken.
Employee was bolting in the face of No. 3 entry on NO. 7 unit. He had drilled the hole for his rib bolt. As he was installing the glue into the hole a piece of slate fell between him and the middle of the entry hitting his head and right shoulder. His neck and shoulder were stiff causing him to miss 2 days.
EE part of the track crew. They were laying track rail (#60) when the EE was prying rails apart the pry bar slipped.
Employee was in rock truck. When rock was loaded, the truck rocked and he struck his elbow off steering wheel.
Repairing a metal stopping between #1 and #2 entry at 36 xc on longwall 60 and the roof behind the longwall shields. This was the first fall of a new face and the gust of air created caused the metal stopping to fall, knocking the metal stopping over. Injured was knocked into a pump-able crib and sustained a concussion and cervical strain.
EE was walking through a crosscut and stepped on ledge and twisted his ankle.
Employee was in the intake air course. He was flipping some rock out of his roadway when he felt a pain in his lower back and right leg.
Employee was in Pitman man bucket being moved into position to perform work when he hit his chest against the bucket brake controller. Employee was wearing fall protection.
While attempting to re-hang a pinner cable, the roof bolter moved up in the next place over, not knowing the cable reel was locked. Cable came down, knocking employee to the ground. Incident only, finished shift. Wasn't reported to Safety Dept until 05/15/2012 Hernia surgery on 6/19/2012 making this a lost time accident.
Employee was exiting his ram car, when his foot landed on uneven ground, as he spun out of the ram car his foot remained planted as his body turned causing a spiral fracture to his left lower leg.
Employee was traveling up a hill in a pick-up truck and hit a large hole in the road. It shook the employee and caused whiplash. The employee was taken to the Harlan ARH. He has a bulging disc in his neck.
Employee twisted his right knee getting on man trip while checking the deep mine pumps.
While operating the continuous miner, a piece of rock flew back striking the employee on right side of neck.
While shoveling mud and muck at the ball mill containment area, employee felt a sharp pain in the center of his back. He was fatigued and possibly slightly dehydrated. He was diagnosed with a muscle strain.
Running a come-a-long, the tooth broke on the machine making him fall, injuring neck and left shoulder.
MINER FELT FOREIGN DEBRIS IN HIS LEFT EYE
While employee was performing pre-shift inspection of equipment he had climbed up truck ladder checking things. As he was coming back down the ladder, he put his foot down on uneven ground & rolled his Left Ankle falling to the ground.
Employee was stepping from pusharm of #320 dozer to track when his foot slipped and left knee came down on pusharm of dozer.
Working on Loader, pinched elbow between H40 Cylinder and bucket arm while changing an O ring on hydraulic hose.
EE was riding the hoist car down the sloper when the brakes locked up and jammed his back against the seat. He went to Logan General where x-rays were negative and was released to work. Due to continuing pain, EE visited Dr on 5/16/12 and was put off work until 5/23/12. On 5/23/12 EE visited Dr again and was excused from work until 6/1/12.
Employee stated as he was unplugging a pump cable from a Utah box he received a electrical shock and it caused him to fall back on the floor and hit the back of his head.
EE was bolting in the return on 16HG. He was installing supplemental support for the future long wall belt entry. He was lowering the drill head from the roof when his left hand got caught between the drill pot and a piece of drill steel laying in the tray resulting in a laceration to his left hand.
The employee was struck on tip on nose by pin wrench (24'). He continued to work and missed the 11th (Family Health Care Assoc). ""As requested by MSHA, the following information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV"".
Employee was installing a gear box. The gear box slid off the shaft and pinched the employees finger causing a cut on the finger.
The employee hanging reflectors with hand on bolter canopy struck by 2"" x 8"" x 1"" rock. FA on fingers. ""As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV"".
EE was kneeling down and lifted a piece of power cable a short distance onto the basket of a cable horse. EE felt pain in his back after lifting. A lower back muscle was strained and required therapy.
Employee was dismounting haul truck, foot got caught causing him to fall. He tried to catch his fall with arm and hurt arm. Went for X-ray on 5/8/12, results indicate wrist is broken.
Employee was using a saw to cut material when it kicked up. Employee claims that he strained his right shoulder.
Employee was spot bolting,he bent down putting his knee on the mine floor,hitting his knee on a rock.Without knowing he stated his knee pad had slid down off of his knee.He went to the ER and it was treated.
Employee was stepping onto an overcast wall and floor that was suspended by a forklift. There was pressure on the fork of the lift against the rib. When the employee stepped onto the overcast, the fork dislodged and sprung upwards against his leg, causing bruising and laceration to his penis. Did not seek medical attention until next day.
While getting off the water truck, the employee stated that when he got to the last step his knee gave way causing him to slip and fall.
Fell while walking landing on left elbow. Reported on 5/8/12. First day missed 5/9/12.
Climbing off ladder on equipment and twisted left knee.
Roof bolter had just finished drilling a hole for 12' cable bolt, while starting to retrieve his steels some of them got stuck in the roof. The bolter then placed his left hand on the drill head and started to insert a steel into the roof with his rt. hand. Before he got the steel into the roof the remaining steels fell out striking his lt. hand. He was wearing metacarpal gloves.
Employees were cleaning up around conveyor, which consisted of throwing some rocks off the pile. Said employee came around to where the rocks were being thrown off and got hit in the back of the leg with a rock.
Starting weed eater while pulling on pull start his left hand slipped off hitting tool shed wall.
Employee had been operating the stockpile loader 5/7/12 and 5/8/12. He called in the morning of 5/9/12 that his back was hurting and needed to go to the doctor. He has a low back strain.
He was sitting on a jeep that was bumped by a motor and duster that had slid out of a switch with a grade. Started missing work on 5/17/12.
Employee was ripping rock with a dozer and when he went to back up the left side track dropped in a 3 foot hole causing a jolt to the operator.
Installing 12' cable roof bolts in BMX 3 East right mains section. Began to install a bolt, it became lodged halfway up the drilled hole. He tried to pull the bolt out and struck left wrist against square plate causing a laceration.
Was loading shuttle car when a piece of rock fell striking his right foot.
Employee had been welding on the canopy retainer rings on the shield job for North River. When employee moved the wire-welder, he felt a pain in his lower back and right shoulder.
Miner was exiting mill office coming down steps and twisted ankle on last step. Visit to doctor and x-rays showed that right ankle was broken.
Employee strained his left shoulder carrying a 15 HP Pump.
on date above employee was removing belt structure on longwall headgate and felt pain in his right shoulder. He worked until 09/10/2012 at which time he had surgery.
Employee hit the back of his head on the corner post of the man trip when a water line he was carrying came loose and struck him on the right side of the head. He received 7 staples in the back of his head.
EE was roof bolting when he bent over to pick up a roof bolt he felt a pain in his stomach.
While lifting a belt roller to load in a uni-hauler, turned felt pain or a pop in lower back. He worked his next two scheduled days when scheduled to come back to work on the May 12th he called stating he could not work because of the injury. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency was a factor.
The employee was engaged in building a seal. In attempting to cut a hole in a Kennedy panel he struck his right thumb with a hammer.
EE stated, while attempting to take a measurement to replace a section of pipe, injured placed his hand on the pipe causing the pipe to fall striking the EE in the back and rt ankle. (This will be a lost time)
Employee was working on a tail shaft using a pry bar. The pry bar slipped and the bar hit him on his left shoulder.
Alleged injury was to have occurred while this contract labor was lifting waterline and he attempted to turn while he was bent over, exerting pressure on his back. Cause of this injury was not proper lifting techniques and enough help. Compliance with rules/regs not a factor, mining equip/systems or PPE a factor and job skills and miner proficiency may be a factor.
While working on the raw coal belt skirt board EE stepped off the belt structure onto a lump of coal twisting his right knee. He told the supervisor around 11:30 during lunch. He finished out the shift. He went to the ER at Montgomery General Hospital around 8:00 that evening, he was told that he had torn ligaments in the knee and to see his family Dr for a follow up.
Employee was traveling in mantrip when it hit the rib and his knee hit the dashboard. Employee went to Gibson General hospital where they ordered him to remain off work until he was seen by an orthopedic surgeon. Employee will be off work for an uncertain period of time.
EE was climbing an 8ft. ladder when the ladder slid downward and his left leg fell through the ladder and landed on the ladder with his left leg underneath the ladder step. This accident was not reported to the shift supervisor at the time it happened, it was not until the 8th of May in the morning when he stepped on a rock that he asked to leave work to go to the clinic.
Employee was stepping off of cage when he fell, landing on his left knee. Employee has been working since date of accident. EMPLOYEE DID NOT START LOSING TIME UNTIL 11/21/12 DUE TO SURGERY ON LEFT KNEE.

Employee was using a slate bar to move the track when the bar slipped and came back hitting him in the side of the face causing a fracture of the zeugmatic arch.
On January 14, 2013, the mine's safety department was notified that the employee had began missing work. The employee had twisted his left knee as he was getting out of a personnel carrier on May 4, 2012. The employee did not miss any work until January 8, 2013.
Employee was a loadout hatcher. While closing the hatch lid of a truck, the driver rolled forward smashing the employee's ankle in-between the truck and the retractable stair system on silo. The employee broke a bone in his right ankle.
Employee alleges he was grinding clips on a belt and a piece of metal got into his right eye.
Employee stated that he was carrying a crib block in the #3 heading of the 15 Left longwall when he tripped and fell on a rock causing him to fall forward landing on his right hand resulting in a fracture.
Employee pulled up to dump area driving Komatsu 785 truck #2527 turning 120' from dump area and backed up at a high rate of speed powering through berm. Material was 70 inches on either side of berm where truck breached the berm, traveling down 30' flipped on its top.
In the process of digging a sump ditch, employee bent over to pick up a pick axe. When he was standing up, he twisted at the waist and felt a pain.
Injured was using a cutting disc with 4"" grinder. Grinder kicked back and cut injured's left forearm.
Employee stepped down after hanging tubing into a hole and twisted his ankle.
Employee was changing screens on Screener and misstepped off catwalk and twisted his ankle.
EE was patching rougher tub on the back side with Nordback. The welder put some material on and picked up the pail and stepped backwards into the tials launder area of the tub. He twisted his right knee when his leg dropped in the launder. Started PT on 05/18/2012.
The SC operator was making a right turn into the #3 heading when ee over steered SC & struck a row of timbers. The ee alleged that he moved over & raised up in his seat to avoid being hit by the timbers & struck his head on the mine roof. The ee had 12"" of clearance over his hard hat. This report is being submitted under protest. The ee resigned on 5/7/12.(attachment to follow)
EE was swinging a maul trying to close a valve. He experienced pain in his right shoulder. Pain increased. He was diagnosed with Multi directional Instability. His work duties were restricted on 5/14/12. Noranda could not accommodate these restrictions, therefore, as of 5/15/12 is now a Lost Time Accident.
Excavator operator was stepping out of his excavator and missed the handhold while climbing off of his machine. Operator has a smashed ACL in his knee and two broken ribs. His knee was injured when he missed the handhold and fell to the ground landing on his feet. His ribs were cracked when he stumbled forward and fell on the track of the machine.
Employee was putting a joint of pipe on fire deluge at No.8 head. Slipped and hurt his right knee.
Employee was burning off bolts from plant screen with oxy-acetylene torch when hot slag bounced onto pant leg. Burned through pants and fell into boot causing 2nd degree burns to left ankle.
Scraping mud from roadway with shovel, felt pain in Rt. shoulder. The accident was May 03, 2012, employee did not miss work until his scheduled surgery, July 3, 2012.
Employee was examining the slope belt which is inclined and slipped on the beltline floor.

He was lifting longwall flights when he felt pain in his lower back. *The operator disputes the allegations in citation #8055888 and submits this report solely to terminate the citation.
Employee was starting to seal a stopping. He was kneeling down and starting to apply plaster, when a rock measuring 20"" X 26"" X 5"" fell about seven feet. The rock struck employee on the head. The roof control and spacing was good.
EE apparently got something in her left eye while working in scale house weighing trucks.

Employee got out of his vehicle, took a couple of steps and tripped on his loose shoe strings and fell. He tried to catch himself with his left hand, causing all his weight to shift to his left hand and arm. Nature of illness, broken wrist.
EE was walking down a side of hill next to conveyor. EE slipped in mud and reached out to grab conveyor to prevent fall. EE fell to ground anyway and dislocated left shoulder.

While conducting pipe-fitting work, EE struck his right elbow on a 10 inch pipe. Experienced elbow and shoulder pain. On 5/16/12, EE placed on restricted duty.
Miner was installing bolt when glove got caught on roof bolt head & he started to rotate bolt at the same time.
Employee bumped his elbow on storage tray on the roofbolter on May 1st. On May 8th during his seven days off he called in and reported the accident. He came back and worked his four days and went on days off. On May 15th employee went to the doctor and was diagnosed with bursitis. Employee went on MLOA FROM May 18TH through May 20th and had a full release to full duty on May 21st.
He was working on the mainline track. He was working off of a monorail car. He was leaning against the side of the car when he leaned back and his neck made contact with the energized trolley wire.
Employee was doing permissibility on a pump. While wading through the mud he twisted his knee. This became reportable on 7/26/12. He is going for therapy.
Employee was traveling down the slope belt entry on a rubber tire 4-wheeler when the chains broke on the suspended belt structure causing one side of the structure to fall. The structure struck the roll bar of the 4-wheeler and the employees left shoulder and head causing swelling and bruising. He was transported to ER by EMS.
EE was attempting to throw a manual track switch that had a lot of tension on it. He was using his foot in an effort to securely latch the switch throw lever. His foot slipped off the lever allowing it to backlash striking him on his left knee.
The tailpiece has a lot of rock that had fell off belt due to narrow belt and rib rolls - lot of water, causing employee to slip and sprain his right foot. Employee did not go for treatment until 5/17/2012 and it was determined that he had a hairline fracture. EMPLOYEE STARTED LOSING TIME 5/22/2012.
Working on spray bar w/pipe wrench, wrench slipped pinching finger between wrench & metal plate.
The employee alleged he strained his back bending a roof bolt on 4 section during the non-producing owl shift. Medical received on 05/14/2012 states spondylosis and lumbar lordosis which suggest presence of an illness rather than an acute process.
Rib rolled off corner of pillar striking car driver in lower back.
EE was trying to move the portable screen. The back end of the loader came off the ground. The loader came down and the EE was bounced inside cab, and hit his head. He was wearing his seat belt.
Employee was using a 5 foot crowbar to maneuver the eye of a dump cylinder which was hoisted on an overhead crane. The employee stood on an elevated platform 22 inches off of the ground as he applied force. He stumbled off of the platform when the eye moved, placing his left hand down to break the fall resulting in a possible fracture to the left radius.
Employee manually turned a full 55-gal drum of oil on a containment pallet so that the pump would fit into the opening. He experienced immediate sharp pain in the back. Employee was seen by physician on 5/31/12 and diagnosed with lumbar strain. After an MRI was performed, employee has been prescribed a treatment plan of physical therapy along with pain block to begin 06/29/12.
Strained back but doesn't know how or when.
A piece of draw rock measuring 53 inches long, 35 inches in width, from 0 to 4 inches thick struck a miner. The injury caused a broken leg and dislocation from the left foot.
Piece of sandstone fell out of the top, employee moved slightly and the sandstone grazed his neck, shoulder and back.
EE was cleaning steel beams through a floor grating with a ""makeshift"" L-shape bar. He was knocking scale from a piece of plywood when the bar went through the plywood and the bar hit the grating pinching his fingers between the bar and grating.EE lost his left thumb nail and broke the first joint on his left middle finger.
Employee was using a come-a-long to remove belt strips from rollers that had become entangled when the come-a-long slipped from the support structure and struck him.
While lifting a trashcan full of shop waste a ""pop"" sound was heard from the right elbow/bicep area of the miner's arm.
Emp. was cleaning out PreHeat Tower vessel with high pressure water blaster gun. Lrg. chunk of matl. fell striking gun causing emp. to fall backwards, pulling gun out of vessel as emp. fell. Trigger struck on gun causing it to go out of control/high pressure water struck emp. in lower leg. Laceration/stitches.
EE was roof bolting. The glove on his left hand got caught in the chuck/drill pot. This cut his little finger (7 stitches) and fractured three fingers (little, ring, middle) on his left hand.
Removing cement chunks from withdrawal silo 55. In the kneeling position. Stood up, started to walk down the stairs, felt pop in his left knee. He felt that his knee would be ok in a couple of days. He stated he wanted to see the doctor on 6/18/12. No restrictions at or medications at that time. Dr. visit of 7/10/12 prescribed medication and therapy with no restrictions.
Placed right hand on mast as it was descending and smashed right middle finger lacerating it.
05-24-12 Employee was moving stone and felt a pain in his back. He did not report and continued working thru the day. 05-25-12 Employee worked thru the day and did not report. 05-30-12 Employee came to work and reported injury from 05/24/12 and requested to go to doctor. He was immediately sent to doctor and then went home for the day.
Doing normal work duties, as the day when on he complained with his eye getting irritated and redness to the eye. He used first aid eye wash to treat at the time. Next morning right eye was swollen shut and he went to the doctor. The doctor did not find anything in his eye, but said he had infection in right eye. Treated with medication.
On May 23rd, 2012 employee radioed his superintendent that he was having chest pains & trouble breathing. He asked to be taken to the emergency room, which he was. There he was given oral nitroglycerin and morphine. The super stayed until girlfriend arrived. This point on were told by EE this was personal med problem and not WC. On 6/15/12 his attorney told us was work related.
Employee was trying to open outby set of doors when he felt pain in his left leg.
I was changing out the screens on the screener and also had to dig out the Main conveyor belt twice after restarting the plant. My lower back hurt causing pain to radiate down my right leg.
employee was walking on the deck of a drill and tripped, twisting his knee. employee sought further medical treatment several days later and was classified as restricted duty on 6-4-12.
EE was cleaning the filter press filters. The electrician was changing a fuse on the bombay doors. EE was with the electrician while he changed the fuse. The electrician started the door and thought EE was beside him. EE had got onto the bombay door to check the hole when the bombay door moved and trapped his right foot between the door and gutter.
Employee picking mud from conveyor line, S & G plant left shoulder became sore. Reported to supervisor but no medical was immediately sought. On 08/20/2012 the employee requested to see doctor, shoulder was sore. First doctor's appointment 08/22/2012. Shoulder previously injured. There has been no lost time and employee continues regular job. Surgery is being considered.
Employee was changing a fuse in a de-energized electrical panel when an arc flash occurred. Employee received burns to hands, chin, and forearms.
While he was helping to install a belt drive he was climbing on the drive and twisted his knee.
Employee strained left knee while cleaning equipment with common water hose.
Transporting a 1"" thick steel plate with forklift. He parked the forklift next to the stands that was going to hold up the sheet. He then grabbed the sheet and started to slide it off the forks on to the support stands. The pulling motion caused stiffness and pain in his mid back region.
EE was operating a chain saw cutting trees. A tree kicked back sideways and struck EE on left knee. MRI shown torn ligament. Surgery is being scheduled to repair ligament.
Employee was removing a chute liner plate. When he removed the plate, the door of the chute came down and hit his arm

While hooking up air hose to grease Jaw Crusher a rock flipped off and hit brim of hard hat, breaking brim and fracturing upper jaw, (no jaw surgery required)affecting nasal passage on that side of face and cutting lips.
EE was climbing into the back of a grease truck when he felt something pop or move between his shoulder blades. He was pulling himself up using his Rt arm/hand to grip a handhold above his head. He experienced immediate pain in his spine and numbness in his right arm and hand. Went to Dr on 5/17/12 and went back to work no restrictions. Went back to Dr on 5/22/12.
Employee was attempting to take out slack from a ratchet strap. The employee tugged with force, and the strap came free causing employee to fall backwards onto his left shoulder.
At approximately 0500 this morning, an operator twisted his ankle at the gallery chute on 484 rail level. No first aid was required and he continued to work on his normal duties. Employee worked his shift then went to the Dr. the next day, x-rays revealed a fracture to his ankle.
Removing cable from barge timber head and caught right hand between cable and timber head when river current changed due to wind or high water. This resulted in crushing fractures to the third and fourth fingers of the right hand.
While removing lug nuts from a haul truck wheel the employees thumb got caught between the lug nut tool and the wheel causing a crush injury.
The injured employee was holding a ""punch"" with a pair of channel locks, to knock out a pin on a take-up jack. Another employee with a hammer was striking the pin. The employee with the hammer missed the punch and struck the injured employee in the right thumb. Both employees were wearing approved eye and hand protection.
The Dozer Operator was working the slope when he trammed over a rock which resulted in the operator being twisted causing a back sprain.

Employee was removing a rub rail from the left continuous miner at the demister cover so he could access the demister. When he pulled the pin from the rub rail the rail fell to the ground and smashed his right middle finger.
Employee was cutting grass with zero turn lawn mower. Tire dropped in ditch causing mower to turn on its side. Roll bar prevented mower from turning completely over. Employee stuck arm out to stop roll over and strained his shoulder.

Operator was cleaning the grizzley at the crusher, backed up and lost his footing. Operator reached for gate with right arm to stop from falling, resulting in right shoulder dislocation. Operator was on restricted duty since 5/14/12. He had surgery on 6/7/13 resulting in a lost time accident.
Employee was in the man basket hanging 480 cable on roof bolts. He started to feel sore during the day. That night the pain got worse and the next day he went to a doctor.
Employee was changing a motor and the motor fell on his finger cutting the tip and breaking the bone.
Employee was attempting to unplug lime chute, while opening the inspection door, lime came down and coated his clothing and portions of his skin. Employee delayed in getting the lime off his clothing and skin. this resulted in 2nd degree burns to both his right foot and left hand.
Changing rollers at the Secondary crusher area on the 2-A (S-3) conveyor. While removing old roller frame, twisted left shoulder, treated at WorkNet for shoulder injury from time of accident. Went for arthroscopic surgery 7-30-12, had been working up until now.
EE was dismounting CAT 980 G Loader in prep. to grease machine. During dismount EE's rt. foot slipped off 2nd to last step and fell backwards to the ground. EE extended lt. arm behind him to break impact of his fall. Resulting in a fractured wrist. This will require EE to have surgery on left wrist on 5/16/12.

REDLINE - Contractor employee (South Industries) slipped, but caught himself while carrying rebar. Later stated his knee was bothering him. Employee went to doctor 5/10, X-ray's and prescription medication were given. Missed two days work. Accident was brought to our attention on 5/11.
EE reported he slipped, when he was working on a 4 foot step ladder. This was reported on 5-8-2012 but refused medical treatment. I was contacted on 5-9-2012 by EE and he requested medical treatment at RedIMed. EE complained of pain in his Right Elbow and forearm.
The I.E. bolting glove was caught between his bolting wrench and the drill chuck on the #29 Roof Bolter when he engaged the rotation. His left arm became entangled with the wrench. He sustained a closed fracture to his left ulna and radius.
Ran over a rock while operating a battery scoop. The jolt bounced him into the top of the cab,jaring his back. ( Complaining of upper and middle back pain)
Victim and scoop operator were moving a bucket liner of bolter supplies. Bucket liner started to come out of bucket, so victim flagged scoop operator and got in bucket liner to hook up a chain. The scoop operator started to travel inby and as he went over a roll in the bottom, the scoop bucket raised up and crushed the victim against the mine roof.
Employee slipped when stepping off the lower rung of ladder when checking belt line conveyor. The slip happened at ground level causing employee to twist knee.
Miner using jackleg to put roof bolts in the back, rock came out of back slid down jackleg hitting ""L"" little finger and left forearm.
Employee was using a shovel working with concrete when the shovel hung up on a bolt in the drive pad, causing him to slip and twist his left knee. reportable lost time injury as of 5/17/12 due to surgery.
The employee was descending the ladder of his haul truck when he felt a sharp pain in his left shoulder. He immediately reported the injury and was examined by a doctor. It was diagnosed as a slight strain and the employee was put on light duty until further examination.
05/07/2012-2:00pm EE reported that he had hurt his back at 7:30am when he removed a rock from top of miner. He complained of pain in his lower back on right side; and he wanted to go to the doctor. Appointment was set for 05/09. After his appointment, he stated that he might have ""pulled a muscle"" and needed an MRI. Per Doctor, he returned on light duty on 05/11/2012.
EE was checking water levels in the scoop battery when the lid fell onto his left wrist/arm. Lost time injury
Employee was changing a drill bit when his left hand got caught between the deck wrench and the bit causing a contusion/abrasion on his left hand.
Employee was walking in the entry, stepped in a hole and twisted his left knee. Reportable lost time injury on 5/16/12 due to surgery.
Employee put his right foot on a truck fender, and then felt a ""pop"" in his left knee.
Helping change screen in the plant. Picked up roll of screen and felt pop & pain in left shoulder. On restricted work activity but can still operate dredge. Taking anti-inflammatory meds.
In attempting to remove a shield from the recovery face the shield was lowered to the transport position. When the shield was lowered the side seal broke loose and fell. The side seal struck the employees right knee.
EE was filling sacks of pellets in the pellet plant. He was hanging the empty sacks on sack rack in the pellet plant. he said he got pain in his right shoulder that continued to worsen throughout the shift. It eventually got so bad he could not continue working. EE had reported some pain in shoulder to the shift foreman earlier in the shift and said having pain for a month.
Truck backing struck drill steel that was extending from a parked truck causing the drill steel to strike the employee.
Unloading cribs - pulling crib off top of stack, it slipped out of hand, he tried to catch it and crib smashed right thumb.
Mechanic was fixing a set of D11 Side frames that were hanging on a crane. Mechanic started to rotate frame with crane and tried to stop side frame with his right hand. He wasn't able to stop swinging motion of side frame and his hand was squished between frame and metal box, breaking right hand.
The vehicle was left in gear and the clutch was not fully depressed when the ignition was applied and the truck jerked forward pinning employee between the rear wheel and the creeper he was on. The rear wheel did not pass over him. He was pinched in between the wheel and the creeper resulting in a broken pelvis.
Employee was working off a ladder using 100% fall protection anchored overhead to a piece of structural steel. His foot slipped off ladder and when land yard caught him his left foot struck and adjacent motor on the side of the foot. The EE was wearing metatarsal footwear which was confirmed during investigation but hit the side of his foot.
Employee was driving a wedge with a hammer to loosen concaves on primary crusher when the wedge released and struck the employee on the back of the leg.
Employee was trying to release the anchor rope on the Guppy Boat when he lost the slack in the rope severing the tip of his left index finger.
Employee was driving a holdown wedge on a shaft timber when the axe that he was using glanced off the rock wall and struck him on his left pinky finger, causing a fracture.
Employee lifting and pushing rock on chopper and rollers.
Employee was cleaning under the lateral 2 head drive with a shovel when a clump of mud slid down and hit him in the knee. The mud caused a bruise on his knee. The injury is a sprained or bruised knee. He was given a brace and took 3 days off.
Employee turned off air valve on the mucker & climbed on top to untangle pump hose. The cable must of hit the lever and caused the bucket to hit his thigh.
Employee was cleaning the dust rings on the crusher when he cut his finger on the blades in the ring.
Employee stepped on a partially covered inspection door and fell back hurting his left knee. Employee incident occurred on 05/31/2012. First aid measures were administered. After further medical evaluation, treating physician put him on restricted duties on 06/28/2012.
While working to remove a 2"" threaded connection EE felt pain in upper back/neck area. EE was using a 3/4"" impact in tight, confined space. EE was sent to the mine medical facility for evaluation. EE sent to outside physician on date of incident. EE placed on restricted duty on 6/1/12.
Employee was dismounting the haul truck when his foot slipped and bent his left leg awkwardly, injuring his knee.
Installing earth tooth on auger using a 3lb hammer. Piece of tooth broke off hitting face and cutting eyebrow.
Employee states that while grinding on a 777 cab, the grinder kicked back and struck his right hand.
A third shift utility crew was dismantling a conveyor belt take-up unit. The ee attempted to block up the unit to allow a scoop bucket to be positioned under the unit. The take-up inadvertently slid off the scoop bucket resulting in the ee's right hand being caught-between a metal block and a piece of angle iron located on the mine floor.
Employee was stacking tires in his truck, when stepping over the tires his foot slipped spraining his l knee.
Was trying to remove fitting from a hot air hose and was cut by exposed wire in hose.
Chipping away buildup in a silo to remove chunks. Multiple locations, angles and positions. August 13 21012 the investigation on this matter was closed and the injury was deemed a new claim and not an aggravation of a previous injury
Employee was removing rocks that were lodged in a crusher when a rock slid out struck him on the right forearm.
Employee was getting ready to hone a piece of stone. He realized his grinder was unplugged. He then plugged the grinder in and the paddle switch was stuck. This led to the grinder taking off and catching the employee on the left arm causing 2 small lacerations.
Employee was dumping oversize material with FEL when right front tire sunk in mud. Employee's hand slipped off steering and was caught by spoke resulting in his wrist twisting & causing slight pain. On June 15 employee's wrist still hurt & he was seen by company doctor and was diagnosed with hairline fracture of bone in his hand.
Employee was assisting with the installation of connector plates in K2. Left pinky and index fingers were caught between connector plate and the stationary heat exchanger. As a result pinky nail was lacerated requiring sutures to close, and was fractured at the first joint. Index finger was bruised and was noted without further injury.
EE stated he picked up chock blocks which were chocking a tire on the truck he was driving & felt pain in his back. This incident has been under investigation by his employer, FlexSource, & their worker' compensation carrier since 5/2912 to determine if this was an on-the-job injury. On 6/19/12, FlexSource was informed by the w/c that this incident occurred on-the-job.
Worker strained lower back while setting up 40' extension ladder.
Repairing water hose. Hand slipped and cut palm of hand on hose clamp.
Alleged he fell and injured rt hip and knee. Did not report until 8-3. Began seeing the specialist in Sept. He was placed on restrictions that we just became aware of. Reached MMI on 11-5-12.
Employee was attempting to lift a oxygen bottle by the lid, the lid came off and hit him in the upper lip.
Employee was hanging high voltage cable when his thumb got caught between a cable hanger and the high voltage cable.
Employee had just finished cleaning C-10 chute and he started to climb out of chute, bringing his left leg against belt to pull himself out, when felt a pain, but pain went away and he did not report to employer until it started hurting again, 2 days later.
Employee was trying to loosen a bolt, when the bolt broke loose he hit his left knuckle middle finger on a stud.
A Working Foreman suffered a low back strain while manually moving the trailing cable for the Marion 7400 Dragline. The incident occurred on Friday May 25th however the employee did not report or request medical treatment until Tuesday May 29th.
Fx R index finger; new ee was at Hi Cal crusher with supervisor. Supervisor was checking rolls to see if they were free by grabbing the belt & moving back & forth. Ee did the same thing on the other side without being asked to and rolled his finger in the pulley.
Employee was kneeling for an extended period of time doing maintenance on a jaw of a crusher changing a jaw die. Initially there was no medical treatment required. On 5/31/12 the employee asked to seek medical treatment for a sore/swollen right knee. He was scheduled for an appointment for 6/1/12. He is now working under restricted work duty.
While climbing up the ladder of a front end loader he fell approximately 3-4 feet on his right hand. The hand is fractured and he has been placed on light duty.
Employee was cleaning out the wobbler when a 12"" rock rolled down a 6' slope and smashed his finger against a 3/8"" plate on a lifting basket. Employee sustained a fracture on his left ring finger, 2 cm laceration to the left ring finger, Contusion to left middle finger & 1 cm laceration to the left middle finger & sutures.
Employee was fueling equipment and tripped over a 2"" fuel hose. He fell and caught himself with his right arm and heard a pop in the shoulder area. ER physician diagnosed with muscle strain and the arm was placed in a sling. He is to follow up with his physician the week of 6/4/12. He is on modified duty until he sees another physician.
The employee was lifting some belt material when he felt a pain in his right arm. The employee continued to work at full capacity until the employee was placed on modified duty on 8/6/2012.
Employee was removing a steam exhaust fan (wt 200lbs) from duct work. As it was being lifted, it ""rolled"", the employee attempted to stabilize it and at this point his finger got caught between the fan motor and the hand railing. He was evaluated by a Physician, diagnosed with a fracture and is working with restrictions.
EMPLOYEE WAS HELPING TO RE-RAIL A RAIL ROAD CAR. RE-RAILING EQUIPMENT WAS BEING PUT INTO PLACE AND RAIL ROAD SPIKES WERE DRIVEN INTO RAILROAD TIES. AS EMPLOYEE WAS DRIVING A SPIKE THE SPIKE CAME OUT OF TIE AND STRUCK EMPLOYEE IN LEFT INDEX FINGER
EE was moving a chute on a dolly. They had to take the chute off the dolly to maneuver it in a tight spot. While moving the chute it fell over and caught his hand between the chute and a handrail. Restrictions were given.
Employee was troubleshooting an air leak on a small air cylinder. When he engaged the cylinder his finger got pinched between the cylinder head and a proximity switch.
Employee was tramming a scoop in a slanted area and it slid into the rib. He complained of his back and neck hurting. He was put on restricted duty for two days and returned to his regular job.
Employee cut right elbow on a piece of steel while doing maintenance work on a rock crusher. The employee needed to have 3 or 4 stitches to the laceration. The employee was released to return to work with restrictions of no lifting, pushing, pulling over 10 lbs. Keep wound clean & dry and limited use of R arm.
Employee was lifting a heavy equipment tire that was mounted on a dayton rim from the ground. The tire was lying flat on hard ground. Employee was lifting one side of tire to get it vertical to roll to the mobile shop when he felt a twinge in his groin area.
EE was exiting heavy equipment (rock breaker Komatsu 200) at the end of his shift on Tuesday May 22nd around 6:30p.m. EE stepped from the tread of the equipment onto the step when the step broke causing the EE to contact the tread/broken step with his lower right leg. EE did not report injury to supervision until Wednesday May 30th.
The employee was treated at the company clinic on 5/22/12 and was placed on restricted duty. While exiting the water truck, the employee failed to maintain 3 points of contact. This resulted in a fall and a fracture to her left wrist.
Employee was stacking a brattice up and as he was installing a door a small part of the brattice fell and hit him. Employee is on restricted duty and the days of restriction will be reported when he returns to full duty work.
EE was stacking 50# bags in the Pellet Plant. He got pain in his left elbow. He could not continue to stack bags. After he reported the injury he told another EE that he started having pain in his elbow the previous Friday when he was operating the 101 packer in the plant.
EE was removing a piece of stuck core from core shore. Sustained puncture wound to L hand.
As stated by miner, shoulder discomfort has occurred to both shoulders, over a reported period of 10 years. Issue began gradual, but with worsening discomfort, over the last six years. This is the first time miner/powderman has officially sought diagnosis/treatment for reported issues. States after loading production holes for explosive detonation, shoulder hurts at end of day!

EE's were putting the crusher back together after changing liners. EE was checking the space on the main shaft when the inner liner shifted and dropped pinching his fingers.
EE was in the process of lowering a metal cover on a miner and as he dropped it into place a cover fell smashing his left hand between the miner frame and the cover causing a laceration and a fracture to his left thumb.
Employee was attempting to close a railcar door latch. The latch wouldn't go down to the locking position. Employee then, using heel of his foot, kicked the latch to close completely.On 5/22 employee received x-ray for foot being swolle,diagnosed with inflammation in foot.

While the employee was mixing mud on top of the mud deck, he removed a bucket of Quik Trol that was covering the bung access hole in the grating of the mud tank. The employee moved the empty bucket out of the way; this removed the cover on the access hole. The helper mixed mud when his right foot slipped into the access hole. He then lost his balance and twisted his ankle.
Employee was taking a caustic sample at a scale trap when the scale broke free allowing caustic to be discharge form the scale trap. The discharged caustic splashed onto the employees groin area. Result was a caustic burn to the groin area. Treating physician placed EE on restricted duty on 05/25/12.
EE tripped on a hose and twisted his left ankle. Diagnosis is left ankle sprain, EE placed on restricted duty on 05/23/2012.
While attempting to install a new tail pulley in a Thor conveyor the employee's finger was caught between the bearing housing and the frame of the conveyor.
Injured employee was attempting to shut a chute on a gate using a sledge hammer. Employee missed the gate and hit his right foot causing a contusion.
Greasing the bobcat bucket when the bucket came off striking his right foot.
Employee lifted a 60# vacuum truck hose resulting in strain to sacrum/coccyx.
While raising a spout for the mobile conveyor one of the bolts broke and it caused the crank to pivot on the bolt that was still in place slamming the back of his hand into the frame of the mobile conveyor.
Employee was repairing hard line when he lost his balance and contacted the rib with his left forearm and elbow.
While water blasting in riser located at preheater tower, material fell down on tool jerking it upward. His hand was pushed upward by the handle of the water blaster tool into an air cannon nearby that smashed his finger.
Employee was removing ash material from lime kiln. A large piece of ash material became lodged in the man way opening. EE tried to dislodge the material from the opening by placing his left hand between the ash material & the metal manway opening. While doing this his left hand became lodged causing him to smash his left little finger.
Employee was picking up an extension cord in the maintenance shop. When he turned around he bumped his knee into a table, and his knee started to swell up.
Employee was using a hydraulic wrench when it slipped and landed on his right hand.
While taking mill samples employee was lifting his 22 lb. sample bucket to take to the Met Lab and twisted his back the wrong way, causing pain in his lower back. He was taken to the ER, given and restricted duty.
Employee removed a lid from a 55 gallon drum. The internal lip from the barrel had a sharp edge sticking out. The employee was moving the barrel and his hand slipped and his thumb was cut on the sharp edge.
