A roof fall was found on the #7B Panel, #6 Heading, #14 Block. The fall area was 17' x 17' and supported with 12' cable bolts and 6' tension bolts. The are was dangered off with posts. No ventilation, equipment, or personnel were affected by the fall.

A roof fall was found on the #7B Panel, #6 Heading, #16-#20 Block. The fall area was 18' x 380' and was supported by 12' cable bolts and 6' tension bolts. The area was dangered off with posts. No ventilation, equipment, or personnel were affected by the fall.
Grass fire that started on the county road. Fire continued from county road to mine perimeter road on south and east high walls. Fire burned between 800 to 1200 acres.
Roof fall cool hill #2 belt (#3 Entry) between 39 and 40 x-cut. 10 feet high - 30 feet long - 18 feet wide.
#9 elevator was inoperable due to the machine break fault, stranding two men on the elevator at 12:28 PM. The men were off the elevator cage at 1:40 PM.
At the above mentioned time an un-planned roof fall occurred in the secondary escapeway for the #2, and #3 section. The fall began 15' outby spad 26, and 90' outby spad 25. The fall continued approximately 75' inby toward spad 38. All approaches were timbered and dangered off.
An accidental roof fall occurred at # 17 block, West Mains rubber tired haul road, and was discovered by mine personnel. The roof fell approx. 12 to 14 feet above the coal seam, 16 feet in length, and approx. 30 feet long. The fall area had been roof bolted & rib bolted to plan. This roof fall will be cleaned up & re-supported.
Fire and smoke was reported from Belt 1 drive tower to the Control Room at ~1:55PM. The control room called 911. All ee's were evacuated and accounted for. Fire depts arrived ~2:15PM. The MSHA Call Center was notified at 2:19PM. MSHA Inspector made contact at ~2:45PM. The fire was declared ""out"" at ~5:00PM. There were no injuries.
After cleaning the power card for the #1 Service Hoist, the power card failed after power was reset. Once the power card was replaced the #1 Service Hoist was restored and operational
While hauling supplies along #4 travelway a large bag of rockdust contacted a steel beam that had been set in the area for additional roof support. This dislodged the beam and allowed a piece of rock measuring approximately 9 foot wide, 13 foot long, and 4 foot thick to fall, preventing travel through area.
An unintentional roof fall occured in the face of the #6 entry on Rattlesnake section at #61 cross-cut on Rattlesnake #2 belt. The fall measured approximately 55 ft long x 20 ft wide x 8 ft high. The surrounding area was bolted with 4 ft resin bolts.
West skip was going down the shaft with the hoist running in automatic.The hoist tripped on overspeed.When hoist tripped off the hoist brakes set causing the W. skip to bounce setting the safety dogs. The hoist operator did not follow policy and cont. up with the skip to the dump. This caused the W. hoist rope to spool off and become entangled on the W. skip.
An unidentified roof fall was discovered at 26xc, #4 entry, of the Z-right Section Panel. The fall was approximately 40' in length x 18' in width x 8' in height. The fall was immediately reported to the regulatory agencies, via telephone hot line. The fall area was dangered off.
Coal was smoldering and was spotted at 8:42 AM. Steps were taken to extinguish the smoldering coal. The coal fire was extinguished at approximately 9:40 AM.
A fall of the roof occurred on the 6th S.E. LOM Mains B x-cut 140-141 (belt). Measuring 25' length by 20' width by 5'-7' height.
Roof Fall in the #9 Seam at crosscut 25, spad +2000 in the #1 entry of the 1st West Sub Mains. Fall was approximately 6' High x 9' wide x 12' long. Fall was between cribs, Ventilation was not affected and no one was trapped or hurt.
An unplanned roof fall occurred at spad #3016, #5 entry on 8 Belt. Ventilation and passage of persons were not affected. Area was initially developed in 2008 and was supported by 5 foot torque tensions. The length of fall was 30ft x 18ft wide and 3ft high. Area will not be rehabilitated and all intersections have been timbered off. Federal and State agencies were notified.
A roof fall occurred on the North East #5 entry at 38 room. The fall was approximately 20 feet long x 9 feet high. No one was injured, the ventilation and escapeways were not affected. The fall was dangered off and posts were set around the fall.
Fall was found on New Haven Mains #5 entry, 2 block panel, just inby spad #1603. Dimensions were 50' long X 18' Wide X 4' thick. Roof support was 4' glue bolts.
At 4:45 pm a ground fault arrestor in the surface substation blew causing a power outage to the west side of the mine. At this time both #1 and #3 hoists were still operational. 4:52 pm the call was made to send crews home rather than have them sit idle. 4:53 pm MSHA notified of power outage. 6:30 pm all crews out of mine. 6:50 pm power restored to mine.
A roof fall was found in #6 intake entry 1 brk. inby SS #847. Area was timbered and dangered off.
While conducting his examination a fireboss identified a roof fall on the Old 1st Right Section off Detour Mains one break inby spad #929. The fall measured approx. 20'L X 30'W X 4'-5'H and was supported with 4' fully grouted resin bolts. The fall will not be cleaned up timbers have been set and the area dangered off.
At 10:50 pm the service hoist failed to operate in that the (sensor overspeed) had water in it making the shaft itself inoperative. All personnel in the mine was withdrawn, and both regulatory agencies were contacted in a timely manner.
An unplanned roof fall occurred in Roadfork #51 Mine near spad#1019 in the #2 entry that measured approx. 60'l. x 20'w. x 6'h. No injuries were reported, the area was developed on 4/15/2010.
The area of shot coal was restricted from access to all mine site personnel because of a K order that was issued on 07/25/12. The shot coal spontaneously combusted and started to produce flames on 08/17/12.
While conducting his weekly exam a fireboss identified a roof fall on the Old 1st Right Section off of Detour Mains near spad #970. The fall measured approx. 20'W X 150'L X 4'-5'H and was supported with 4' fully grouted resin bolts. The fall will not be cleaned up and timbers have been set and the area dangered off.
THE THIRD SHIFT MINE FOREMAN DISCOVERED ROOF FALL ON SUPPLY ROAD IN THE FIRST SOUTH PANEL APPROX. 1000' FROM # 1 UNIT.
The #1 Elevator would stop 1 foot short of the landing. The car would not return to the top. EE found dirt in the bottom door track preventing the bottom door from closing all the way. This caused a poor connection of the bottom door lock. EE cleaned the bottom door track and the door lock contacts. The car was tested and returned to service at 7:19pm
AN UNPLANNED ROOF FALL OCCURRED AT X-CUT # 8 ON THE 1 SOUTH BELT. THE FALL WAS ABOUT 20 FOOT LONG, 15 FOOT WIDE, AND 4 TO 6 FOOT HIGH. THE FALL WAS TIMBER AND DANGERED OFF. A PLAN WAS APPROVED AND THE FALL WAS CLEANED UP AND A CANOPY PUT IN IT.
Sub-surface cavity collapsed beneath dragline causing it to sink into cavity with a twisting motion causing substantial damage to the structure of the machine
While conducting his weekly exam a fireboss identified a roof fall near spad #9998 at the mouth of the Old Mead Section. The fall measured approx. 20'W X 30'L X 4'-5'H and was supported by 4' fully grouted resin bolts. The fall will not be cleaned up and timbers set and the area dangered off.

Elevator stopped due to the door switches on the cage needing to be reset. Door switches were reset.
11:04 pm the mine lost power due to a blown lightning arrestor in the surface substation. 11:15 pm the call was made to evacuate the mine. 11:16 pm MSHA was notified. #3 and #2 hoist were used on generator power to hoist personnel from the mine. All employees were evacuated by 1:16 am. MSHA was made aware at 1:19 am. Power was restored to the mine by 4:30 am.
An unintentional roof fall occurred at the 4W Headgate Entry #2, 4840 W. The fall was approx. 27' L. x approx 18' W. and of undetermined thickness. The fall did not affect ventilation. The fall will be cleaned up. Fall#29C
A roof fall occurred in the bleeder entry of the I-O longwall panel. The fall occurred in a 3 way intersection that is located in the #1 entry at crosscut 35 and the fall measured approximately 20' long by 30' wide by 4' in height.
WHILE TRAVELING DOWN THE NSM TRAVEL WAY EMPLOYEE CAME UPON A ROOF FALL IN THE TRAVEL WAY AT X-CUT 44.
The Crabapple Elevator was inoperable for more than 30 Min. No one was on board, no injuries as a result of this incident.
Roof fall discovered by mine examiner on the 1-Right Beltline extending from just outby SS#628 to SS#835 (540' x 19' x 4' to 12' high).
A roof fall was discovered by fireboss inby Survey Station # 155 on mine slope, size of fall 6"" W X 18' L X 6' H. Roof fall will be cleaned up, does not affect ventilation
AN UNPLANNED ROOF FALL OCCURRED AT X-CUT # 33 ON THE OLD WAY TRACK. THE FALL WAS ABOUT 50 FOOT LONG, 20 FOOT WIDE, AND 6 TO 8 FOOT HIGH. THE FALL WAS TIMBER AND DANGERED OFF AND WILL BE CLEANED UP AT A LATER DATE.
A neighboring facility operator struck a power line knocking power to our facility. All employees evacuated the mine under a non-emergency conditions by way of our Koepe Hoist. Power was restored at approximately 9:00 am and employees reentered the mine at approximately 10:00 am (we let the main fan run for 30 mins prior to reentering) There were no injuries or damage.
A fire was discovered by Production Control Supervisor in the 001 Pan Conveyor Gallery at 22:00. The PCS called the local fire department at 22:10 due to amount of fire and smoke in the gallery.
Pond 1135 experienced a failure of the west wall with approximately 9 inches of tailings and water leaving the pond, traveling through a stormwater conveyance ditch, filling a sediment basin and leaving site through the rip rap outfall.
A fall of the mine roof was discovered in the 9 hg ""C"" panel at #5 crosscut in the #4 entry. The fall measured approximately 35' long X 20' wide X 10' thick. The roof fall does not affect ventilation or travel and has been timbered off.
An unintentional fall of the mine roof was discovered at #12 c.c. #3 entry of the South Mains Belt. The fall was approximately 50' long x 18 ' wide x 5' high. The fall was immediately reported to the regulatory agencies, via telephone hot line. A roof fall clean up plan will be submitted.
At 1:57 pm power was lost due to a blown lightning resistor in the surface substation. 2:05pm the decision was made to evacuate the mine, 2:08 pm MSHA was notified. #3 hoist was used on generator power to hoist employees out of the mine. All employees out by 3:35 pm. 4:14 pm power was restored to the mine.
An unplanned roof fall occurred on the #6 belt at break #16. No persons were affected
Surface crack fall is located 20 feet inby 16 Bk to 5 feet from inby corner of 17 Bk (Spad 55) on mains. Fall was approx 70 feet long 6 feet thick on outby end, 5 feet thick on inby end, fall was a s wide as entry on bottom and 10 feet wide on top.
After taking the first lift to the left in the No. 6 entry, the continuous miner was backed out of the lift the bolted roof in the entry fell approximately 7 to 8 feet high, entrapping the continuous miner and 2 MRS's. The unbolted roof in the lift was not immediately affected.
The crane was being moved into position to make a lift on a containment tub when the cranes boom came into contact with some overhead power lines. Employee stayed in the crane until it was confirmed that the power was off the lines, he was not injured and was checked out at a local medical facility to confirm there were no injuries to him.
The #8 raw coal feeder became blocked and a void was created. The dozer operator was in the process of cutting to the void when he trammed too close to the void's edge and the dozer he was operating slipped into the void.
A roof fall was discovered above the anchorage in 5 North #1 entry just outby #6 crosscut. No injuries. Ventilation and travel are not affected. Fall is 70' long x 16' wide x 12' high. Additional support has been set on both sided of the fall.
An unintentional roof fall was discovered at approximately 10:15 a.m. at brk. 20, No.2 Hdg, on the Grassy Creek Mains. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up. The fall was approximately 8'-10' high, 20' wide, and approximately 60' long. The fall was in the intersection.
Examiner was traveling with MSHA inspector when they found a rock fall in the 2NE Parallel at spad # 23+80 (xc34) in the No.3-Intake entry. They were able to set over in the No.4-entry.
Roof fall has occurred one entry over from spad #2218 outby the 2B overcast in the intake in the 1st left southwest parallel. Fall was 20 feet long, 6 feet height and 16 feet in width. The fall will be cleaned up.

an unplanned roof fall occurred in the #4 entry head end of 4 right panel 1 break inby the 13404 spad. This fall is a continuation of a previous fall that occurred on 3/9/12 e-document number 1535764.
At 6:00 pm a supervisor notices a brush pile fire that was getting out of control and at that time the fire became unplanned. The supervisor contacts dozers and water trucks by radio to help extinguish and control the fire. The supervisor then calls Safety and it was determined that MSHA needed to be called because the fire would be lasting beyond 30 minutes.
Roof fall in intersection at spad No. 4372. Approximately 30' long; 20' wide; 6' to 8'.
Longwall headgate deemed impassable due to deteriorated roof conditions. A roof fall did not occur, but additional support was required for persons to pass through area safely.
The auxiliary contact for the main contact on the Metz elevator failed causing an entrapment of 12 people for 45 minutes.
The Blake's Ridge Elevator stopped with 7 men aboard. We experienced an unplanned power outage which tripped a breaker on the BODE Air Compressor. The breaker was reset and the elevator ran normal.
An unintentional roof fall was discovered at approx 10:45am at brk 77 on Grassy Creek mains. All approaches to the fall were timbered off. Fall did not interrupt ventilation, will not be cleaned up. Fall was approx 6-8' high, 20' wide & approx 143' long. Fall was halfway between 6 & 7 entry, thu the intersection of #7, up #8 entry for approx 15'. The fall is Y shaped.
Elevator was down due to an electrical problem.
The Prod hoist tripped and could not be reset. Upon investigation It was determined that an H strand on the SW guide rope of the west skip had separated from the rope and had coiled up between the shoeboxes for the entire length of the rope. The coiled H strand contacted and damaged the first limit switch as the skip reached the head frame.
#1 Elevator was out of service from 3:39 PM to 6:30 PM, no individuals were on the elevator at the time. Schindler was notified and adjusted/cleaned the K-switches. The elevator was back in service at 6:30 PM.
Kink was discovered in slope car hoisting cable. Hoist was no usable until 8/4/12 3:40am
A roof fall was discovered on the 1- North Mains. the roof fall is located in the #5 entry and crosscut at break #5. The roof fall has been dangered off. The fall has not affected the ventilation at this mine. The fall will not be cleaned up. The approx. dimensions of the fall is 20' w. x 20-30' l. x 6' thick.
Broken strands were found in the hoist rope at Crabapple slope, which caused the hoist to be out of service for an extended period.
On 8-2-12 lightning caused wildfires encroached on mine property burning appx 672 acres. Defensive lines were built around mine to prevent spread onto mine property. Mining operations were unaffected, fire didn't spread into pits or other active mine areas. MSHA was notified & verbal 103(1) order was issued. When MSHA arrived, order was modified to a 103(k) & terminated.
A non-injury fall of rib/brow at 35 x-cut along the D mains Section return air course. The fall was not above the anchor bolts. The fall did effect the ventilation and escapeway by damaging an overcast.
A face ignition occurred on the E-2 Longwall at 12:10pm. The power was de-energized and all government agencies were notified.
An unplanned roof fall was found by the examiner while traveling in air course. It is located in 2nd parallel at spad #4+90 in the No.3 Intake entry. It did not affect ventilation.
A fatal accident occurred at the Exodus underground mine on Friday August 31st at approximately 13:30. The employee was preparing the top cut of the 4410-315 long hole stope, when he fell into the stope.
Employee was atop of cement silo when the roof collapsed engulfing him.
EE stated he was unwinding a spool of elec. cable. Reel was suspended from the roof cable became fouled causing reel to swing back towards EE's face he threw up his right arm to catch reel when his ring finger was caught by the chain holding the cable reel and severing the end of his ring finger back to the first joint.
Employee was descending a service loader with a filter assembly. The assembly got stuck on the steps and the employees right middle and ring finger tips were snipped off by a pinch point created between the handle and the housing.
While getting out of the basket on the tower truck, employee's ring on his left ring finger caught on the basket, ripping the skin loose all around his finger.
While servicing a water pump on a highway drill, a faulty hydraulic motor engaged while the driller was inspecting the belts and pulleys. His three fingers (index, middle and ring) were trapped between the belt and pulley severing the tips to the first knuckle.
While cutting out the equalizer pin bar, the left side track frame on # 542 D10R dozer came down crushing the right leg of the affected employee. Investigation in process.
The employee was installing a cable bolts when he experienced pain in his groin. The employee began missing work on 8/27/2012.
Employee was picking rock from rock table and another rock hit his finger.
Employee struck thumb when placing screen on screen deck.
The miner was shoveling sand since early morning and was working to clean up sand near a walkway and hurt his lower back.
Ee was walking beside a belt line when he tripped and fell. On the way down, his hand hit a belt roller and broke a bone in his hand. With proper medical care and therapy, his hand healed well and he is back to work no restrictions.
Pulling pin to release dust car pulled pin car moved and pinched finger.
Employee was picking up oxygen tank with cap and the cap came off when employee picked it up and it struck him in the mouth. Employee had surgery on 10/9/12 when he started losing time.
Employee reported pain in groin area while operating dozer. Diagnosed with a hernia. Compensability not yet determined.
While picking up rock dust bags to load the wet duster, he felt sharp pain in lower back. ***He continued to work full-time without restrictions until 12-10-12.***
Employee was pinning ribs and rib came off and struck his hand against truss bolter jaws.
EE walked into a red zone and was struck by the boom of the miner and rib.

A crew miners was working on the Cedar Rapids crusher when the crusher needed to be cleaned, one employee shut sown the power. Before the rotary stopped turning, EE stuck a pry-bar in and the bar came back out and hit him in the head.
Ee was greasing a drive line when the coupling came loose and shot grease into his hand. The grease actually penetrated his skin. He knew to see doctor's care immediately. With antibiotics his hand healed very well while he only missed one day.
EE was cleaning ash underneath the belt.EE attempted to use a fox tail brush,the belt idler caught his glove pulling his hand into a pinch point.EE pulled his hand out,had received severe compression trauma.EE called control room 911 called,EE's crew responded and first aid given to stop the bleeding.At 5:24 a.m.the ambulance arrived,EE was transported to E.R.See Attached
Employee stepped off motor on uneven ground and twisted his ankle.
The injured and another employee were attempting to move a screen deck with a forklift and nylon strap. The forklift operator went to pick one end of the deck up, the strap broke and the deck fell and smashed the other employee's foot.
Employee was picking up wedges, when a loose rock fell and hit his left leg
The employee stated he strained his back lifting on rock at the 3A belt head while cleaning the area. The incident has resulted in lost time.
Replacing a hydraulic hose on the #2 continuous miner employee was pulling on the hose to get it in the correct location. At that time his hand slipped while pulling the hose and struck himself in the right rib.
Employee was cleaning in the unit 4 tail area, when she heard a rib crack. She took off running and fell down onto the ground. She landed on her right arm leaving a small cut on it. She was released to come back to work but did not come back because she is experiencing dizziness.
Employee was checking anti-freeze in radiator of DIIR Dozer when he took cap off, the liquid went onto his face it was hot.
While moving the feeder wood fell and struck employee on the neck.
EE was backing cat 777 haul truck to rock dump, truck went over dump and slid or rolled approx. 150' to bottom of pit.
The employee moved approximately 20 bags of gel when he thought he felt something odd in his back muscles. He stood straight up to stretch his back and that is when he felt discomfort in his abdomen and notice it bulging.
While working on replacing an electric motor at the Primary Crusher the employee apparently grabbed or pulled on the Come-A-Long causing it to fall off a beam and struck the employee in the nose, breaking it.
Employee was taking down a roof bolter cable and when it came down it struck him on the chin causing him to fall backwards, landing on his rescuer / tools.
Tripped carrying water cooler strained Left shoulder.
Employee was unloading supply's off of a rail car when he tripped over the rail and twisted his right ankle.
Normal working section. Hard bottom, crawling and twisting on his knee.
EE was hauling coal to feeder with shuttle car, he hit his forehead on the fly pad, knocking his head back and cutting the back of head.
Turning off two inch water line valve and felt pain in his left shoulder. He began losing time his next regularly scheduled shift.
While Fire bossing the slope, employee stepped on a rock and fell twisting her right knee.
Employee was attempting to scale, when he felt something pop in his shoulder. On 02/08/2013, the employee underwent surgery to repair inflammation in his shoulder.
Employee was disconnecting battery leads to work on contactors of a Mantrip. While holding the lid up on the batteries, the lid slipped and fell onto his left forearm.
The injured was roofbolting on 3 East Mains. The injured was dropping the drill mast on the roofbolter when the drill mast caught a long wrench, causing the wrench to flip and hit him in the right side of the face. The injured sustained a crushing injury to his right eye socket.
Operator pulled up to office trailer to unload his personnel objects & bin controllers when he climb out to ladder and started down was probably about 4 feet from ground he lost his grip when shoe got stuck on step tread-and fell backwards, he stated that he was only using a couple of finger to hold on with items in hand.
Twisted knee getting out of scoop-worker thought his knee would be ok, did not see doctor till 9/12/12. Doctor's diagnosis was sprain knee. Therapy did not help. After MRI was done a tear was found in meniscus. Scheduled for surgery 10/18/12.
Emloyee got out of the mantrip at #2 head where the #1 section was located. He was walking toward his toolbox when he stepped on a rock and twisted his right knee. The area is well rockdusted, dry and lights around. Employee continued to work his shift and refused medical treatment. After work he drove to Raleigh General and was given seven days away from work.
The injured was operating the roof bolter when a rock fell from the top striking his lower back. The injured wasn't diagnosed until 9/12/2012.
Employee was moving rock dust bags and felt pain in back
Employee reached back over his right shoulder and felt a twitch in his right shoulder.
Employee was attempting to fuel a gas-powered pressure washer. Engine caught fire burning the underside of his arms causing second degree burns.
EE was unloading structure off of a forklift when a piece of rail fell off and hit him on top of his left foot bruising it.
Employee was putting resin into split set, when the tube ""popped"" open & squirted resin over the top of his safety glasses & hardhat, getting into his eyes. Went to medic. Then went to Doctor- who issued restrictions.
EE was putting pressure to a roofbolt after spinning when a piece of rock fell cutting his arm.
On 8/28/2012 employee was shoveling gypsum, which weighs approximately 15 lbs per shovel into a loader buckets. During this time, the employee seemed to pull/sprain a muscle or tendon in his elbow region. It seems to be a very minor injury. Employee did return to work during the ten day, however, went back on disablity.
EE was cleaning Prep Plant floors and steps, started down a set of steps while cleaning with water, the third step from the bottom had a bend in the step on one side. He slipped on it, fell on left side. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor. Became a lost time injury on 9/13 2012.
On 8/27/12 the employee was cut on his elbow when the nut he was trying to loosen broke free & his arm hit the frame of a machine. He received 1st aid & returned to work. On 8/28/12, while using a wrench to turn a thread-tapper & compensating for the pain in his elbow, the wrench slipped, glanced off his safety glasses & struck him in the forehead, causing another laceration.
Employee was taking buggy anchor down from where it was hung and it slipped. When he caught it something popped in his back.
Employee was stepping off buggy and twisted his knee, employee felt pain immediately. Further medical evaluation determined surgery was needed. Became recordable on day of surgery, 9-9-12 DAFW.
Employee was assigned to flush on the 400 and 500 MFL pads and twisted her left ankle while walking to the valves.
Employee slipped on muddy bulldozer tracks getting off machine, twisting and hitting knee. Employee did not miss any work. 10/11/12 received note stating operator should wear elastic brace while working. 10/26/12 received note with a diagnosis of a meniscus tear and a planned surgery. We determined we should report after this diagnosis. Employee has not missed work.
Employee was pulling rock dust hose that was tangled. As he was pulling the hose it caught on drive support and when it came loose, he slipped in wet rock, dust and mud injuring his lower back. His first date of loss is 9/13/2012.

Employee was sent to the cooler electrical room for cleaning and maintenance purpose. When finished the job he passes in front of the cooler hydraulic room that had tree leaves and mud in the floor he slipped but not fell and the force made to avoid the fall cause the right leg muscle to strain.
OPERATOR MISSED STEP WHILE CLIMBING DOWN FROM THE CAB OF LOADER AND FELL TO THE GROUND.
While bolting, employee felt a ""pop"" in his lower back, causing pain & discomfort. He went to medic on site. We took to doctor and restrictions were issued.

Employee bypassed guard on belt conveyor to clear sand built up around the tail pulley. Employee failed to de-energize equipment and perform Lockout/Tagout procedures. The shovel made contact with the belt, it pulled the shovel into the tail pulley. When the shovel was pulled into the tail pulley it began to spin striking the employees left arm with the shovel handle.
Employee was operating a D6T dozer to build a new road above the existing road when the outside bank gave away and the dozer rolled over the bank. Employee was initially treated for a concussion but has not returned to work, exact injuries unknown at this time, but he is off work and undergoing physical therapy for his neck and knee.
Employees were setting timbers when a piece of loose rock fell hitting the employee on the head and back
Miner working in the refinery, noticed that the drain to a sump, located under a vessel, was not fully closed. The miner, not realizing the depth of the hot solution, stepped into the sump and the water went over his knee high boots and burned his left leg.
The operator pulled into the mine and noticed that there was some spillage that needed to be cleaned up. He then parked the haul wagon, got into the loader and proceeded to clean the mess up. While cleaning the mess he noticed that he was getting closer to the haul wagon when he ran into it cause some damage to the cat walk and hand rail on the haul wagon and nothing on the loader.
Employee injured became a lost injury for this accident on 2/1/2013. Employee slipped when he was prying with a bar on brake car and fell both knees struck brake car. PPE was not a factor, compliances rules/regs was not a factor, mining equipment was not a factor, miner knowledge and proficiency was a factor.
Dozer op. decided on his own that he was working 3:1 slope on hollow fill. The op. was told that he wasn't to get on the slope, but he did anyway. Op. backed too far down slope and went off a washout area on the next slope. Inexperienced op. caused the accident.
In process of putting piece of metal on a machine. Machine was part of Wash Plant and had a chute on the back of it. The metal piece slipped and dropped on right calf. Calf was lacerated
EE stated he cut 9 face and back up to cut 9 right. While cutting 9 right piece of rock fell out between pins striking left leg.
Employee was prying with a pry bar when he got his thumb pinched between pry bar and pin on the miner. Accident was not reported until 9/19/12.
Employee was in underground warehouse unpacking new mucker seats. While lifting, he experienced pain in the chest area. Diagnosis: Muscle Strain. work restrictions were imposed
While moving a jackleg, he lost his grip on it & it fell with the D-handle striking the backside of his knee, which then made his knee strike a rung on a ladder causing discomfort.
EE was notified at approx. 4:35 p.m. he was being laid off. At approx 4:40p.m. he wanted to report an accident. EE was operating a 672 John Deere Grader and reports he hit a rock in the haulroad and hurt his back and neck at approx 9:00 a.m. After reporting the incident he proceeded to leave the job site. See Attachment.
The injured party was working at the #35 crosscut of the number 2 belt. He was attempting to throw a wooden pallet across the belt and felt a pop in his back. He completed this shift and returned to work on 08/27/2012 and he completed this shift. On 08/28/2012 employee called in and stated he was in pain and was going to see a doctor.
EE was helping electrician splice cable when he slipped and fell.
Injured was assisting co-worker on center bolting. Employee did not realize that the steel was still in the roof. After removing driver steel the finisher steel unlodged itself from the roof falling and striking the employee in the right pinky finger knuckle.
He was rolling a rock and ended up pinching his 1st, 2nd and 3rd finger tips on his left hand under the rock.
Employee was changing a tire on a coal hauler. He was walking the tire in place by twisting it side to side. He felt a pain in his lower back, that extended to his left leg. He had surgery on 04/02/2013, causing this to be a lost time.
Employee standing on one end of stone barge tied to the dock where loading takes place was struck by the winch cable that moves the barge when the cable clamps were unable to hold cable at the cable eye. Employee sustained a simple fracture of the fibula and swelling of area due to trauma requiring a procedure to relieve pressure.
Employee was installing an outside bolt. As he went to get a pizza pan to put on the bolt, he slipped on a rock and twisted his right knee.
Employee was oiling chains on booms of Fletcher Drill Number 7. While stepping down off of drill employee tripped on bed hose causing him to fall against frame of drill, striking left shoulder and elbow. Possible Rotator Cuff tear in shoulder.
The injured was operating #61 continuous miner when a piece of rock measuring 3' X 5' X 6"" thick flipped out of the conveyor striking him on his left leg causing a fracture to the tibia and fibula that required surgery to stabilize. The incident occurred on MMU-003
In #00 Heading, the man was cutting a piece of draw rock 1"" thick by 30"" long 12"" wide fell from the top hit the man on his head and shoulders. Knocking him down.
Employee was in the process of rock dusting a belt line. The duster stop for a minute so he sat on a water line that was held up by chains attached to a rib bolt plate. The plate that the water line was hooked to broke and the water line fell landing on his ankle causing a fracture to his lower leg.
The Hoe Operator was loading a large rock onto a Euclid when the rock slipped from the hoe bucket and fell onto the bed of the Euclid, jolting the Euclid driver causing strain to his neck, shoulders and lower back.
Employee was operating shuttle car in #4 entry. He pulled through a curtain, pulled forward and hit back of miner tail boom with shuttle car. Impact with miner boom shook him and his left shoulder and left side started hurting. He should have been watching more closely and making less speed.
Worker was cleaning debris from roller on raw coal belt. Belt was locked out and tagged. As debris was removed, stored tension caused the belt to move trapping the workers right hand and arm between the roller and belt up to his shoulder. Initial injury report is a cracked ulna bone with possible torn ligaments. Further test and injury results pending.
EE was heading outby from unit sub when he tripped over cables or rock. He strained his achilles tendon in his right ankle.

Employee was driving a kubota down the 72 722 ramp, when the kubota tipped on its side. This resulted in a fracture to employees left foot.
While employee was tramming the miner, a piece of draw rock (25"" X 18"" X 0-7"") fell striking the employee on the left foot causing a hairline fracture to the second metatarsal.
Miner was using a four inch grinder with a cutting wheel while cutting plate steel. Blade hung up in seam and shattered cutting miners left index finger. Miner received +/- 15 stitches in finger and was released back to duty.
Employee was helping co-workers physically pull and relocate the power cable for the section ventilation auxiliary fan, when the cable they were pulling on became stuck and came to an abrupt stop, the employee felt pain in his lower abdomen area.
Stated: Employee was assisting mechanic in changing the inside dual on the water truck. He was using a pry bar to lift on the tire and strained his lower back. Diagnosed with low back strain. Placed off work with medication and physical therapy.
Bates Contractor was in the process of advancing belt and power when he strained his back while pulling cables or handling materials.
Submitted under protest to abate citation #8511176. EE struck head on a scoop canopy on 8/21/12. Missed no work until 9/14/12 diagnosed with a normal CT scan and released to RTW with no lite days. Did not mention being injured until 10/11/12, Is currently investigating the claim and reserve the right to amend and/or correct this report.

Employee was loading rock dust bags onto the scoop. Employee stated that when he turned to put the rock dust bag onto the scoop he felt pain in his back and right leg. A diagnosis of a lumbar disc was not made until 9/11/2012.

A bag of portland cement was travel up an incline belt on its side. Miner pushed bag with broom handle to position bag. Broom handle pierced bag and portland cement went around safety glasses and got in employee's right eye. Cornea of right eye is scratched.
Employee was taking apart air line while standing in side of shuttle car moved out of the way of 2nd employee and stepped off shuttle car and fell to the ground and injured right shoulder. Physical Therapy.

Employee was removing a fuel tank cover from a SR-1 man trip when the cover fell hitting his right middle finger.
An electrical motor, wrapped in plastic, was lying on the tailgate of a pick up truck. The top of the plastic was cut and as he lifted the motor to get the plastic from underneath the motor he felt pain in his back. Due to pain in his back he started missing work on 8-23-2012.
Employee was using come a longs to unload shuttle car tire off mantrip. He was pulling on the come a longs when they got wedged, he was standing in mud. When he pulled, the come a longs broke loose causing him to twist his knee.

While installing a guard on the screening plant, EE was walking around plant and in an effort to avoid a water puddle he stepped on a rock which shifted causing EE to twist his right knee.
The miner bent over to close a 6 inch butterfly valve on a discharge line and while bending and twisting to close the valve felt pain in his lower left back area.
Stated: Employee was driving golf cart on travel road and ran over a butt off a timber. The butt lodged under the golf cart and in the road, causing the golf cart to swerve sideways and wreck. It threw him into a gob pile in a XC. He went to the ER and was diagnosed with left shoulder pain, low back pain and fracture of first left rib.
Employee mounted the motor grader using the proper hand holds and walkway. His objective was to clean the right rear side window. While walking on walkway located on the tandems, employee slipped and struck elbow on a tire. Employee was utilizing the grab rail, but lost his grip. The walkway was clean and dry.

Employee was at brk. 24 at 3 left on 002 section building a brattice. Brattice was not secured when it began to fall striking left thumb and back of head. Employee received a fracture to his left thumb from this accident.
Injured employee was using a jack to lift a 9Man man-trip when the jack kicked out and struck the employee on the left foot. He was taken to the ER and diagnosed with a contusion of the left foot and ankle. He was excused from work by the ER physician.
Employee was cleaning the cab tilted the back rest forward, it the process of positioning the back rest back to upright position. Placed the hand to unlock the seat lever and smashed right thu mb with the back rest.
EE was attempting to weld a nut onto a bolt to make a puller. He had just started to weld when he felt the top of his right foot burning. This resulted in a burn to the top of the EE's right foot.
EE allegedly claims head, shoulder, back injury when he was unloading by hand and moving a piece of steel weighing 78 lbs. He stated he caught his left heel causing him to trip and fall resulting in the injuries.
On 8/18/12 EE stated he came into work with his right leg muscle tight, drove 773 and alleged that the pain got worse. In 8/31/12 EE reported as a ""near miss"". On 9/7/12 EE sought medical attention and placed off work as a work related injury.
Employee was loading roof bolts onto a roof bolter. He was putting a bundle of bolts down. A wire from the bundle caught his glove and smashed finger between bolts and roof bolter.

The employee was greasing the belt drive at the preparation plant clean coal silo and tripped on an air hose. The employee fell, lacerating the end of his left middle and ring finger. Sutures were not required, but the physician was concerned about infection and instructed days away from work.
EE was using slate bar to lift a scoop tire to get a chain under the tire. This resulted in pain to his left shoulder. EE continued to work until his surgery on November 26,2012.
EE was traveling down the #2 entry on a battery powered scoop on his way to the charging station. As he crossed under some cable guard that was supported by tie wire. The tie wire hanging down entered his RT eye causing an abrasion.
While rolling a clamp over to a pick-up site, the clamp rolled on uneven ground and fell onto the EE's right foot. The EE was evaluated by a physician on 8/30/12 and was able to continue working full duty. In January 2014 the EE complained of non-resolving right foot pain. He was evaluated by a physican and surgery was performed on 3/13/14.
The employee stated that he was pulling and pushing on a roof bolt as he was inserting the roof bolt into the drill, he felt a burning sensation in his right arm.
Employee was sprayed in both eyes when working around sample box. Employee immediately went to eye wash station and flushed eyes out, then supervisor took and flushed eyes again. Left eye was okay, but right eye had possible abrasions. Flew employee out of camp to Anchorage, AK to be seen in ER. Was flown out Sunday, 08/19/2012.

He was tramming scoop around the stump coal was fluffing of the cover going down his neck. He put his hand up to stop coal from going down his neck and caught his fingers between the coal and canopy of the scoop pinching fingers on left hand.
Injured ee and other ee had an altercation. Injured ee struck by other ee on side of head. Injured ee worked remainder of shift, woke up next morning with increased neck/head pain. Seen at ER and put off work.
Employee was walking on under/ground road way , slipped in rut in road way , which was wet and muddy.
When employee was cleaning the plugged chute a rock fell on his foot above the steel toe and bruised it. Doctor said to stay off it for a day and could return to work on Friday, 8/17/2012. There was no production at the plant so employee did not return until Monday, 8/20/2012.

The employee states he was starting to climb on to a D11 dozer to check the fuel, and his foot slipped causing him to fall on his left shoulder. The employee continued to work until taken off work on August 23, 2012 for physical therapy.
Employee was struck on top of the head by a rock that fell from between two roof support straps while roof bolting.

Working 39yrs.in a coal mine, chronic overuse & the attached Dr. letter stating that repetitive trauma contributed to the knee tear.The EE's 1st day away from work was 11/1/12.
Employee was walking down the stairs, when she reported her ankle gave out on her. Her right ankle was swollen and was taken to the Berkeley Spring War Memorial Hospital. She ended up with a twisted right ankle, with a Fracture Lateral Malleolus-CL.
Standing over top of conveyor - bending over to remove troughing rollers - twisted back wrong. Reported as incident only on 8/15/12 (1st aid). Continued working until 8/22/12 - back hurting more.
Employee was lifting a cabinet into a loader bucket when he injured his bicep. He went to a physician who referred him to an orthopedic specialist.
While filling a fan rock duster with bagged rock dust, the employee felt a pain in his low back. The incident became reportable when the employee began missing work on 08/20/2012.
EE was pulling on a canopy extention on a roof bolter and felt a sharp pain in his groin area. He continued to work his regular duties, but the pain persisted. He sought medical attention with his family doctor on 08/23/2012, at which time he was taken off work for a hernia.
The Roof Bolter operator had installed his first bolt in a face. As he was preparing his second bolt some draw slate fell inby the ATRS in the unbolted area. Apparently a piece of the slate flipped back and struck him on the left leg and hip. He returned to work the next day but started missing on 8/21/12 due to continued pain in the hip area.
Victim was priming a pump and while picking up a 5 gallon bucket of water felt a pull in his neck and right shoulder.
EE was shoveling under feeder. Co-worker noticed conveyor belt was out of place. He went to check it & heard EE yelling for help. He noticed EE's arm was pinned between conveyor belt and roller. He stopped the conveyor. Another co-worker came over & helped drop the roller to free EE's arm from the roller. Resulted in injured rotator cuff & burns to back.
As Employee was holding a piece of belt down to cut a flapper off the knife slipped and cut his left forearm. Employee was treated at Raleigh General Hospital and received four (4) staples.
EE was bolting #3 right on #2 unit when a rock came off of the canopy of the bolter and struck him.
The EE was installing roof bolts in the #7 entry. As the EE was pushing the roof bolt up into the hole a small piece of rock fell striking the EE on the head which resulted in a strain to his neck and shoulder.
While operating a motor grader, the moldboard hit a rock and the employee began to experience back pain. Employee suffers from arthritis and pain coming from the facet joint that is behind the lumbar 4 and 5 vertebrae. Employee did not return to work.
Preparing to bend a piece of steel. Heating the steel to help ease of bending, placed foot on steel to bend once the steel became hot enough, while using force of foot tendon on left foot popped. EE was taken to hospital for medical attention.
Employee was retrieving 4"" pipe from the belt line in16th East and stacking the pipe in xc #16 when he stepped on a board and a nail penetrated thru his rubber boot and the nail punctured the bottom of his left foot. The ER doctor bandaged the wound and advised that he take 1 day off from work.
EE was removing built up dirt and rock debris from a piece of equipment. While using a pick a large piece of rock/dirt fell on his leg.
Employee was unloading a tool box from a 3 wheeler. The tool box slipped and when he tried to catch it, he twisted his back. Employee's first day away from work was 8/21/2012.

A miner was leaning out of the bucket to position a pipe when he decided he needed to be higher. He rang the bells to raise the bucket then failed to get the bells rang to stop the bucket. He was crushed between a guide rail timber and the bucket. The injuries included head lacerations, neck & spinal injuries.
Driving the truck and the seat system failed causing injury to the employees back.

Employee was installing cribs in the 9 seam. He was in a high place and a piece of draw rock fell from between pins striking him on the head/neck. He began missing time on 9-14-12.
Employee was exiting a Jonboat, when he stepped out of the boat his foot sank in the mud and lost his balance, fell and sprained his back.

Miner went to doctor for pain/discomfort in his shoulder and underwent surgery on January 8, 2013. Physician believes the pain/discomfort is linked to either joint degeneration, or a minor shoulder strain miner received on August 13th for which he received first aid and returned to work.
Employee was roof bolting, was bending roof bolt he felt something pull in right shoulder and his hand went numb. Was seen at Medical Center and received e-ray exam of shoulder.
While loading a train an empty railcar was observed with tree limbs inside it. The injured was standing on the ladder on the outside of the stopped car while another employee handed the tree limbs out to him. The injured lost his balance and fell hitting his side on the trailing railcar, then falling to the ground. The injured suffered a cracked rib.
Electrician was pulling old wires from old line 6 fine screen conduit. Wire was overhead and strained his right shoulder. EE had surgery on 03/27/2013.
Descending into mine on the skip hoist, there was an emergency stop of the cage. EE experienced pain, discomfort and soreness. Employee was taken off of work effective 8-13-2012 DAFW.
Employee was walking with two buckets, stepped on a rock, rolling her ankle and falling to the ground. Seen at the ER, given a boot and crutches and put off work for three days.
Crew was on a liner change. EE was unbolting the rec. hopper while standing on the motor stand. His wrench slipped off the bolt he lost his footing and fell backward approx. 42"" down onto the stand deck landing on his left shoulder blade and head. EE went to the ER and examined and released with bruising. Dr. inst.him to stay home a few days, if the pain persisted to get an MRI.
Loading belt structure, bottom rollers, onto a rail car another employee threw a roller which slid and hit his little finger on his left hand. This impact broke his finger.
The employee stated that he was pulling the continuous miner cable when he stepped into a hole causing him to fall and twist his right knee
EE states that he was spraying off belt structure with a water hose; hose behind him got caught in belt causing hose to be pulled into belt conveyor. EE did not release hose and his right arm got pulled into belt between belt roller and belt.
Employee was assisting in the turning over of the stage loader drive assembly. Employee had his hand on the drive as it turned. The drive was approximately 3/4 ways turned over when the unbolted deck plate turned loose, striking employee's left middle finger.
A hi-pressure hose, that was fully engaged caused the EE to be pushed against the long wall face. The EE sustained a dislocated right shoulder and a laceration ti his left hand that required 12 sutures.
Miner was walking past a 2 yard mucker when the mucker articulated and the bumper struck the miner's lower leg resulting in fracture.
The employee stated that he was at the #6 belt drive when he slipped and fell hitting his back on a small pebble which resulted in his lower back hurting.
Employee stated that while using a slate bar to unlatch a beam, the bar slipped out and struck him on the arm.
ON 08/10/12 SUBJECT WORKER WAS STACKING ROCK ONTO A PALLET WHEN THE ROCK SHIFTED AND SLID UP AGAINST HIS RIGHT HAND. THIS RESULTED IN TWO POSSIBLE BROKEN FINGERS ON HIS RIGHT HAND. THEY WERE HIS LITTLE FINGER AND RING FINGER. AFTER HIS EXAMINE THE NEXT DAY. THE E.R. STAFF TOLD HIM THAT HE HAD TWO BROKEN FINGERS. HE WAS TREATED AND RELEASED ON 08/11/12.
Employee was connecting (2) air hose together when one slipped out of his hand and struck him above his right eye.
employee was swapping bulk loading hose from tank 6 over to tank 7. He squatted down to close ears for connection, stood back up and felt a slight release followed by what felt like fluid filling in my lower back. Went to walk to pumper scale house and was starting to feel pain and stiffness with each step.
Employee pinched his left index finger between the end of the bolt wrench and the frame of the drill head while he was roof bolting.
Unknown, employee noticed material in right eye about noon; he went immediately to on-site EMT who flushed out the eye. After 24-hours and no improvement, the employee was flown from camp to Anchorage ER for further treatment.
While working on the lotrac, wrench slipped and struck employee's left wrist against the lotrac. Fracture ulna.
Employee was riding mantrip in when it was struck from behind by another mantrip.
Employee sprain neck while riding out on a manbus. Employee has continued to work since accident until 8/16/12 when taken off work by the doctor.
While operating a dozer, backed over a large rock, which caused pain in lower back
The employee's foot was caught under a supply car, causing him to fall and strain his knee. The employee began missing work on 8/22/2012.
Drawrock fell while EE was changing pinner bits-knocking EE to ground
Employee was stepping up onto form when he became dizzy, lost his balance and fell to the same level, due to high blood pressure. As a result of the fall the employee hit his head on a beam.
EE was walking to back of bolter to tram the bolter from deck due to steep hill and he slipped and tried to catch his self from falling and caught right hand between the light guard and light
While loading the rock dust trailer, he was trying to lift himself up on the trailer with his left arm to pull the plastic off the bag. He felt his right shoulder pop and then give out causing all his weight to come down on it. He immediately was scheduled for a MRI and the results were positive for a tear in his rotator cuff. ***He began missing on 8-14-12 for surgery.***
Employee stated that he traveled over a ""rough"" spot in the haul road going onto the Frost permit and struck his right elbow on something in the cab.
Employee was sitting on motor while EE was mucking with 12b mucker and got sand in his right eye.
While moving cable in the pit at North Shore Mining an employee fell and as a result received a laceration above his right eye and several smaller cuts on his right ear that required sutures.
Skill saw kicked back while cutting steaks for concrete forms cutting right thumb of employee requiring surgery on 8/10/12.
Victim attempted to hold on to bolter wrench while reversing rotation in order to remove a bolt he bent during its installation. The wrench/bolt snagged his glove and pulled him into the steel fracturing his left arm. He was released from hospital same day and is scheduled for surgery later.
EE went to step off a catwalk at the dumping bin. His pants caught on the handle of the forklift wheel chock. This caused him to slip off the catwalk twisting his left ankle.
Shoveling coal to clean-up walkway at feeder and stated that he strained his back.
Employee was dragging a curtain to the face of the #3 entry when a rock fell out between the bolts striking him on his left knee. Employee continued to work until 10-15-2012 when he went off to have a torn meniscus repaired.
Employee experienced a strain of the lower back while exiting skid-steer tractor.
Employee stated that while mounting the fork lift his foot slipped off the third step and he fell to the ground
While changing scoop battery, scoop arm (that holds battery) struck battery that was on the footwall pushing it on to the top of the employee's left foot. Employee's first day off was 8/9/2012.
In the process of dismounting he slipped off the steps and caught himself. When this happened he felt a pull in his lower back.
EE was walking in locked and tagged out face conveyor chain and a piece of rock measuring approx. 132"" l. x 60"" w. x 4"" t. fell in front of shield tips, bounced off coal face and into conveyor chain striking employee.
Injured employee was attempting to place crib blocks under the cutter head of company number 138 continuous miner. While placing the cribs he caught his left thumb between a crib block and the head of the miner resulting in a laceration and fracture. Injured employee finished the shift then received 6 stitches to close the wound.
Employee stepped down to ground level from the auger house stairs and as he went to turn to the right to leave the area, he twisted his left ankle resulting in a fracture.
Moving a piece of steel plate and when he turned to set it down he hit his elbow on a ladder causing muscle tear on the right elbow region
The employee was assisting in pulling a new electrical line for our well. He was bent over and when he stood up his back was tight. No one else was injured. He was wearing all of his PPE.
Driving heavy equipment tractor, reversing, tractor rail caught a slab of rock, jarring motion sprained back requiring chiropractic treatment on 08/08/12
An employee working on a belt conveyor drive motor change misjudged the distance to the floor while stepping off the conveyor, fell backwards, and sustained fractures to both wrists.
Individual entered the rear of a one ton rack body truck containing a steel frame leaning against the East side of the truck rack. Individual was previously advised and trained not to do this. No damage to the truck or frame. Individual sustained a compound fracture to the lower leg.
The employee was roof bolting at the time of the incident. While the employee was cleaning and repairing the suction lines, he placed his left hand over the suction hole. The steel then fell from the roof, striking the employee's hand.
EE was in the process of removing guarding along 8B Conveyor. He felt a pop in his back but continued to work. After dropping a pry bar he bent to pick it up and felt a sharp pain in his back and his upper right thigh and could not straighten up.
Was down in elevator, cleaning ""B"" pan, stepped in water and got lime burn
Employee was unloading empty pallet from back of mechanic's truck and when he stepped to the ground his knee gave out and felt pain. This is the same knee that was injured in 7/2011 requiring surgery where the employee was off until he was released to return to work on 4/9/2012.
Bent to pick up Conspec cable and felt pain in groin area. Employee continued to work until 11/16/12 when he had surgery to repair hernia.
Affected employee was involved installing a scaffold base layout on 3 1/3 floor of a preheater. Suddenly without any warning, part of the refractory wall above the work area failed and pieces of the wall began falling. The refractory pieces fell about thirty feet striking employee causing injuries.
Employee was lifting slider rail with curtain from out of mud/water on footwall when he injured his right elbow. He has been working since injury up until he started losing time on 10/29/2012.
EE hit his knee on a Johnson 3 Wheeler on July 2/, 2012. He refused medical attention at this time. On August 6, 2012, he came forward & requested to go to Doctor, we took him to Dr. He was given (1) day off. He was off work 8/7/12 and returned to work 8/8/12.
Injured employee tripped over a loader bucket tooth and fell to the ground on 08/06/12 injuring his right shoulder. He was seen at the ER on the day of the incident and released to return the work the following day. Due to continued discomfort of the injury, the employee was re-evaluated and excused from work on 11/01/12.

While doing daily check on batteries, he got battery acid on face and in his eyes
Mechanic was removing the wheel cover from a loader when the cover slipped contacting the middle and ring fingers on his right hand and causing a laceration requiring 6 stitches on his middle finger.

The employee was letting down the cable of a MRS and a rope hanger struck him in the right eye.
While changing bags in bag house, with system disabled, cement dust flew into eyes and face from above, causing irritation to the eyes.
Employee was roof bolting, a rock fell from the top between roof bolts and the rib tearing down line curtain. Employee was moving the rock off the curtain when he said that he felt something pull in his lower back.
EE and co-worker were working on feeder jack. Co-worker threw a crib block toward the feeder; the crib block struck EE on his upper left leg causing him to fall onto the jack. EE had pain and bruising to left leg and left side. Transported to hospital for exam, x-rays, etc. No fractures. Discharged to return to full duty based on mobility on left leg.

A production employee lost his balance while entering the cooler. The employee fell and broke the fall with his knee which suffered a cut over the knee.
At approx 3:30 am an MT4400 empty haul truck was found rolled over onto passenger side of the vehicle at the dump. Employee sustained slight fracture to T-1, and some abrasions to arms/face. Employee was transported to Barstow Community Hospital, treated and released.
The employee was dismounting the diesel Wallace D-U-V-14 man ride and stepping in a dip twisting his right ankle. The event has resulted in a lost time injury case.
Employee had just taken receptacle loose from scoop and was getting down from the top of the scoop to put on charge. When he stepped off the scoop he twisted his left ankle.

Stated: Employee was drilling hole for roof bolt. He drilled in his first steel and dropped the arm to connect the second steel to the first. He raised the arm to continue drilling. He did not notice the bottom of the second steel was not in the chuck but on the shoulder. He continued with the upward pressure. The steel slipped off and hit his right hand, fracturing his thumb.
Employee was loading bags of block bond onto a scoop tractor. The scoop bucket was damp. The employee slipped and felt pain in lower back.
On 8-3-12 employee was opening a material door and suffered a torn Achilles tendon. After a medical exam it was determined that sometime prior to opening the door he had injured his tendon, this injury is being submitted as an accumulative trauma type of injury. The time difference between when the injury occurred and the reporting date was pending a medical report.
Employee stated he had an ""Old Back Injury"" and while he was shoveling, he felt a pain in his back.
The mill operator was freeing compacted material from the top of a feed hopper when a large rock dislodged and struck this left leg.
Employee was shoveling ore leakage from under a conveyor belt. He has been treated for a non work related back since May 2012. He shoveled 3-4 shovel fulls when he pulled a muscle in his lower back.
EE was sandblasting and pulling the air hose when something got caught on the ground pulling EE backwards, EE then felt something pop in his lower/mid back. EE did not tell anyone that he had hurt his self until a day later. EE was taken to see the doctor and placed on light duty on 8-7-12.
Employee was getting down from a screw housing and twisted his right knee when stepping onto the floor.

Employee was a passenger in a rubber tired man trip on the way out of the mine, when the front bumper of the man trip contacted the end of the track haulage rail and stopped abruptly, causing the employee to be forced backwards into the passengers seat.
Employee was cleaning coal with a 930 loader when his bucket caught on a sand rock and he twisted his back. He kept working with back pain thinking it may go away, but the pain kept getting worse. On 8/13/12 he thought he was having a kidney stone attack and went to the ER At Harlan ARH. He then called and said it was his back.
Employee was helping a build a brattice while reaching a co-worker a brattice block his right foot got stuck in the mud causing right knee to twist tearing ACL ligament, employee became a lost time on 08/06/12 due to this injury.
Employee was loading roof bolts onto roof bolter when he picked up a hand full of bolts he twisted around and strained his back.
Employee was riding in a scoop bucket when the scoop went over a hump causing the scoop bucket to raise into the top causing a injury to him.
EE was operating bulldozer in front of the 1900 excavator trying to dislodge a large rock. The dozer spun around to the left and shot the rock away under him and he came down with the rock landing on the left side of the dozer.
Operator went to turn and step off the track of the machine and slipped and fell 3 feet to the ground, severely dislocating his left ankle. It was a dry sunny morning, the tracks were dry, the ground was soft as it was freshly excavated topsoil/overburden.
Employee was bagging M-200 mullite product, as he took the bag off the mullite spout to place on the pallet, his arm struck the mullite bagging spout, causing a 1 and 1/2 inch long laceration on his left forearm near the elbow that resulted in 14 stitches.

Employee was scattering belt structure when he experienced low back pain. Employee stated that this may be a reoccurrence of an old Army injury.

Employee was standing along the operator side of his ram car when a rock approximately 3ft. x 3ft. x 2in. fell out from between the rib and rib bolt and struck him on the left side of his head and then fell on top of his left foot. He received a fracture to his left big toe.
He was looking under scoop to check for the operator if something was under scoop and put his right arm between the block on the scoop and the mine roof and the op. raised the bucket caught his right arm between the roof and the block.
The employee stated he pulled groin muscle while getting up from his knees. The company does not believe this injury occurred at the mine. This is to comply with citation 9051012.
He was walking on the longwall face and tripped over a relay bar landing on his right wrist. *We dispute the allegations in Citation #8049158 and submit this report solely to terminate the citation.
Employee was closing hatches on OTR customer truck when the driver drove off with EE still on trailer. EE was forced up against the side rails of the safety platform but hung on and kept from falling to ground. EE suffered sprains to neck/back, contusions to arms, legs and back. EE prescribed RX for pain in the p.m, and phys. therapy 3x wk. No lost time/restricted duty.
Employee was climbing down from the cab of the Peterbilt truck-tractor when he lost his footing and fell backwards to the ground. The employee tried to brace himself with his left arm, subsequently suffering a fracture of his left wrist.
Miner was climbing up stairs and injured his right foot. Miner indicated that he had overstepped one of the stair platforms as he was stepping up the stairs. As the miner continued to climb the stair, the right foot became wedged in-between two sections of the stairs which caused (2) metatarsal bones to break in his right foot.
Employee was walking in quarry and tripped and fell on left shoulder due to uneven surface. Employee has now been diagnosed with a rotator cuff tear. At this point, our employee will continue to work his regular duty job but may have surgery this winter.
Employee was moving T-5 channels (16' long) from one roof bolter to another. Channel slipped out of his hand and he grabbed it. Felt pain in shoulder. Began losing time 9/20/2012.
Mechanic was swinging a 16 pound hammer, when he felt a pop in his left shoulder.
The employee went to pick up a box of bolts and felt a sharp pain in his lower back.
Running 7"" casing collar caught on can, while putting on 7"" plate caught employee hand between table and handle on plate.
This employee refused medical treatment until now. Lost time began 9/13/12 and he will be going to a specialist. He was working with a concrete saw 8/27/12 and alleges injury to his low back.
Heel of left boot was cut off by employee while carrying 2"" air pump. He slipped on grounding mat at DC power center - mat was damp and slippery. He twisted his left knee and was slightly swollen.
EE's were lifting a miner tram idler to load it on the three wheeler at the end of the track. IE felt a sharp pain in his lower back while lifting.
While working in the shop, he tripped on 4 x 4 dunnage and jammed his shoulder trying to break his fall. He has continued to work until last week when he kept complaining of discomfort. He finally went to see a doctor and was diagnosed with a slight tear of a tendon. He underwent surgery this past Friday, February 8th, 2013.
Employee was traveling when encountered approximately 4""- 5"" of water at #38 break. There was material under the water that the employee did not see as he entered the water, as he tripped and fell.
The injured employee was roof bolting in the face of the #1 entry of 10th East section. After drilling a hole, he was attempting to insert a tube of resin into the drilled hole and as he reached above his head he heard a pop and felt a sharp pain in his left shoulder.
EE was directed by the dump operator to dump part of the load on top to repair a low berm. The operator pulled forward to allow the material to flow out, as the bed cleared the berm, the bed raised, then dropped to the level it had been hoisted, jarring the ee. EE was given restrictions until cleared by PCP. Restrictions could not be accommodated. Ongoing.
Employee was pulling on a hose when his foot got caught on another hose on the ground. Employee tripped and landed on hands, resulting in a broken right wrist.
Maintenance crew was repairing FK Pump. No specific movement or event caused pain, back became more sore as the day progressed. Injured has had suffered from frequent back pain, although with this incident, injured has also experienced numbness in the left outer shin and foot in the days following the heavy work. Injured did not seek medical treatment until 09/05/2012.
A 3rd shift crew was assigned to advance the belt one break. They had trammed the feeder out of the way and were in the process of moving the feeder cable when an ee pulling the feeder cable received a shock from a damaged shuttle car cable laying on the muddy mine floor in the #4 crosscut. As a result of the shock the ee fell to the mine floor striking his back on 3 crib blocks.
EE advancing power, hanging HV cable & nip--partner let go of cable, heavy load strained L Shoulder. RTW normal work until diagnostic arthroscopy performed--No damage found, however LWD followed procedure starting 12/03--indirectly related to injury.
Employee was lifting a duel head air pump, and as he tilted the pump to the right side, he got his finger in-between the pump and the belt structure. Employee was wearing gloves.
EE was lifting 20# steel plate when he felt a pull in his right shoulder as he pushed the plate onto a scaffold. Did not become restricted until 09/04/12.
Employee was lifting hood on truck for preshift inspection, the hood was hung on something and the employee pulled harder and felt pain in his pectoral muscle.
Backing scoop and caught left hand between stop and kitchen. Smashed little finger and ring finger.
The employee stated he stepped into a hole twisting his back when walking to the old 3-section checking a pump. The event resulted in a lost time injury.
Employee stepped out a pick up truck and when she started walking she noticed pain in her right knee. No event occurred. Employee was placed on restrictions on 8/24/2012.
EE was lifting stones off of saw table after they had been cut to size.
Employee was stepping out of a boat onto land on 8/21/12 when he twisted his right knee, it was later determined through MRI that he had torn his meniscus. Employee has been working until he had surgery to repair on 10/11/12.
After scaling a loose rock from the back, the miner drilled a hole so he could install a 6' split set bolt. when he went to reposition the jack-leg another rock fell from the back and struck his left hand causing a laceration and a fracture to his hand.
Employee, parked s/c on grade at 16-Block #3 Entry Spad #3868. S/c slid approx. 3' pinning oper. between s/c & rib, employee had set park brake and got off machine to move roof bolter cable.
Mechanic performing work on equipment using a ladder. While twisting his upper body from the ladder, he experienced sharp pain in his lower back that radiated to both legs. He returned to his dorm room at the end of his shift, but was unable to sleep due to pain. He denies hx of back problems. He began losing work on 10/25/2012 due to surgery and has not yet returned to work.
Palatalizing rock from out of the rock bins and turned, which cause employee to pull muscle in his back.
Employee was changing a screen on a screen deck, he was pulling on an angle-iron rail and when the item was dislodged he pinched his thumb on his right hand between the rail and the screen motor. EE suffered a broken bone in his thumb and a contusion to the thumb nail.
Employee was opening the layers of a large stone that was standing on its edge. When it opened, the outside layer fell on the employee's right foot. Restricted duty was assigned.
Employee was working with the roll crusher in the lab. Employee was clearing rocks from the crusher with a rod when the rod was pulled in and pinched her right index finger between the guard and rod. The 1/4"" tip of her right index finger was amputated and her third finger received contusions.
Employee states he was grading roads and inadvertently bumped his hard hat on a roof bolt. Reportable lost time injury 10/5/12 due to surgery.
Cutting metal and hot slag got into his pant leg and down into left shoe burning foot.
Injured was setting up a set of supply doors, one of the doors slipped and fell on his foot.
Drilling a 8' hole and his steel stopped up, he pulled one section of steel out of the roof, placed his right hand over the pot to increase suction, then the other section fell out fracturing his right thumb.
Employee felt and heard a tearing sound from the left shoulder while working the lever back and forth of a 1/4 ton chain come along. Employee was in the act of removing a rod assembly from a hydraulic cylinder. The hydraulic cylinder was laying horizontally in the shop on the cylinder work bench.
EE was using a chain come along to tighten/guide chain holding tubing for Cone Crusher. Chain lost tension snapped hitting EE on the right side of face.
EE states he was riding in a mantrip hit a bump, causing him to strike his head on the roof of the mantrip and bounce back down into the seat, causing pain in his lower back.Reportable lost time injury 10/30/12 due to surgery
Employee was stacking stone when he pinched his finger between 2 stones. Employee did not seek treatment until 8/22/12. Restricted duty assigned.
Miner was getting off haul truck, put weight on left leg and felt/heard a pop. Miner thought it was ok so she got in haul truck and tried to apply brake but was unable to. Miner just returned to work from personal injury to same leg. Workman's Comp was pending due to previous injury and has stated to report it as Restricted Duty Injury due to Strain/Sprain of left knee.
EE was loading airlock doors in scoop bucket and felt sharp pain in left shoulder and arm. NOTE: EE did not begin missing work until 9.6.12
EE was working on a pump, ground was uneven and wet. EE slipped on wet surface and twisted right knee. EE stated he felt and heard a ""pop"". EE report mine site medical facility and was sent off-site for x-rays. On 08/15/12, EE was placed on restricted duty.
Employee was in the process of removing a rock that had hung in 2 C Header. He heard a ""pop"" in his elbow on 8-13-12. He had a MRI on 8-14-12. 8-15-12 he had his follow up on MRI. 8-16-12 MRI revealed torn bicep tendon he was scheduled for surgery on 8-27-12. He started missing work on 8-28-12.
Employee was lifting stones from conveyor belt to put on pallet when he felt a pain in his back & lower neck. Employee did not report injury or seek treatment until 8/14/2012. Restricted duty was assigned.
EE was looking for a dredge cable while walking along an embankment. The soil beneath EE's feet gave way and he fell approximately 6 to 7 feet. EE was the only per involved.
DRILLER HELPER WAS STANDING ON THE FOOT CLAMP AND THEN THE DRILLER LOWERED THE HEAD WITH THE ROD SHOOT ON IT AND THE HEAD CAME DOWN ON THE EMPLOYEES FOOT.
Employee was removing coal from the cable trough on the longwall face when a piece of rock fell off the tip of a shield and struck the back of his right hand causing a fracture.

Heavy lifting may have caused a hernia while performing maintenance tasks at the Rock Plant. The employee did not report the injury to his supervisor until 8/20/12, at which time he was taken immediately to Simi Health Center, where he is currently being referred to a surgeon for treatment. Until then, the employee is on restricted duty with no lifting over 15 lb.
Walking down a descending ramp to his truck, his left foot slipped out from under him causing him to go into a ""split"". His R knee struck the ground with his R foot planted up hill. His lower leg bowed until he heard a ""pop"".

On 8/9/12, employee team lifted a shackle and cables from the ground to a loader tow pin. He reported that he experienced pain in the abdomen. After taking a pain reliever the pain went away. The pain seemed to come and go. On 9/24/12, the employee visited a physician who sent him to see a specialist. On 10/4/2012, the Specialist confirmed a hernia. Surgery is scheduled.
While putting a wall piece into place, pressurized mud blew out, knocking the employee into a tank on the east floor.
Employee was helping in the loading of bolting supplies onto the roof bolter. The tray was partially loaded onto the bolter with the remaining portion resting on the push plate of the scoop. The tray slid off of the push plate pinching employee's finger between winch cable and push plate. His wound required stitches to close and was placed on restricted duty.

The employee was attempting to free up an adjustment piece on a 48"" pipe wrench. A piece of metal broke off when the pipe wrench was hit with a hammer causing the piece of metal to penetrate the skin of the employee's arm.
Injured pulled guard of G1 Tails pump, turned and placed guard on ground. The Employee felt pain in lower back. After 1st visit, the nurse practitioner released employee to return to work with no restrictions. After the 2nd follow-up visit with the nurse, the employee was prescribed prescription medicine and put on restricted duty, due to condition not improving.
While removing concaves (metal liners-1500 lbs each) in the primary crusher bowl, an employee was on the wall of the crusher attempting to loosen a concave. As the concave fell, it rotated out of position contacting the employee's right foot. The employee was transported to Nye Regional Medical Center where it was determined that the employees foot was fractured.

Injured noticed swelling in his right elbow at break time. Injured reported to his supervisor that he must have bumped something during the day that caused the swelling. The following day 8/8, injured requested medical attention when the swelling didn't improve. Injured was diagnosed with bursitis.
Employee reported while operating a dozer, he ran over a rock and hit his head inside the cab. Two days after the incident (8/9), he began complaining about neck pain and clumsiness of the left arm.
Affected employee was involved installing a scaffold base layout on 3-1/3 floor of a preheater. Suddenly without warning, part of the refractory wall above the work area failed and pieces of the wall began falling. The refractory pieces fell about thirty feet striking employee causing injuries.

On 8-6-12 EE was moving a stopper and felt a pain in his knee. He continued to work; on 9/10/12 he was seen by a doctor and was taken off work pending a MRI. On 9-17 he had a MRI. On 11/15/12 he had surgery. The time difference between reporting and submitting report was pending a medical report that determined that this was accepted as an industrial injury.
He reported on 8/13/12 still waiting info from doctors...EE hit rt knee on ladder of haul truck then noticed swelling days later, went to personal dr. and then to ER. On the 13th we sent him to our provider once he notified us.
Due to horseplay, injured EE was pushed, lost his balance & twisted his shoulder as he caught himself. Dr. determined EE had strained shoulder muscle which resulted in 1st aid incident. He was revaluated by an orthopedic surgeon & determined to have a torn rotator cuff. Surgery conducted on 1/17/13. At a follow up on 1/21/13 the dr wouldn't release back to modified work.
Employee was returning from getting water in Maintenance Office. Upon re-entering the loader cab he put his right hand where door closes and utilized his left hand to shut the door. He thought he could move his right hand before the door shut. The door shut on his right middle finger. He reopened the door and removed his finger.
Employee reported pain to lower abdomen after holding and then dropping to the floor a ladle carrying babbitt material. Doctor's 1st Aid. He had 4 scheduled days off (8/3-8/6) following the accident. He returned and complained of pain (8/7). We sent him back to the doctor who then gave him 2 days off (8/8-8/9). Return to work with restrictions.
EE slipped and fell while loading planks for overcast construction and injured his left wrist. Restricted duty as of 8/10/12.
While swinging out the boom on his roof bolter had his hand on the canopy and pinched his left little finger against the coal rib causing a partial amputation. Transported to Memorial Medical Center Springfield where the finger was re-attached.
The employee was in a truck with a crane attached. He was lifting a pump with the crane. The crane fell and jerked the truck up and slammed it down. The employee hurt his back with the impact of the truck when it fell.
Pulling bit up through rig table, bit caught edge of table, bounced back into the table wrench which then struck the employee holding wrench in the abdomen.
Injured was removing section of cast liner in side rod mill. As he was striking the bottom of the casting to break it loose an approx 12""x18""x5"" piece of the top of the liner broke off and fell approx 4 feet striking the employee on the top of his left foot. The piece weighed an estimated 75 lbs. Employee was wearing work boots with metatarsal protection.
While moving furniture to conference room employee attempted to move a hearing booth by himself with a dolly. The booth was to heavy and fell back on his right leg/knee. Employee was then taken to clinic and determined he had sprained his knee.
Injured was adjusting the calibration on a motion switch for H-50 dust collector screw. He accessed the switch from the wrong side of the guard and while descending the ladder his left thumb contacted a sharp edge on the guard.
Repairs to man basket, turn buckle, basket would no go into latch mechanism so used crane to moved into place, technician unhooked crane then got into basket to free latch and basket free 4' then stopped in mid air causing right knee to hit rail in basket and left ear hit rail in basket.
Miner was using Hilti DX 460 powder actuated tool. He jumped down with the tool in his hand and the gun misfired or was unintentionally fired at him. There is a hole in the lower left of the back of his hard hat and he suffered an occipital contusion.
While coming down the spiral staircase, a black cat ran in front of the EE startling him where he missed his footing on the third step, fell forward, and jammed his 5th metacarpal causing a spiral fracture.
A Roof Bolter Operator had a drill steel stuck in the drill chuck and was attempting to pull it out with a chain that was tied off to the centralizer arms. Once the drill steel was in rotating motion the chain spun loose and struck the employee in the face and nose.
Employee was unloading cribbing from a timber skip in the raise and staging it beside him to move into the stope. This material is approximately 5' long and weighs 30 - 40 lbs. As he unloaded a piece he felt a pain in his back which was diagnosed as a sprain.
The employee bent to pick up the guard to replace on tail pulley after cleaning. When he lifted the guard from the ground he strained his back.
Operator was walking across the deck of a haul truck and tripped on a bolt falling on her right knee and hand.

EE was walking up the incline walkway when he slipped and fell on his right knee. EE was evaluated by onsite medical provider and sent an off site medical provider for further evaluation. EE placed on restricted duty on 8/29/12 by off site provider. EE returned to work full duty on 9/5/12.
The employee was placing stone onto a pallet when his thumb was smashed between two pieces of stone. He was taking to a clinic where he received sutures and prescriptions.
In response to Part 50 Audit 2-5-14. Leach pad operator slipped while carrying tool box down slope.
Using a 1/2"" electric drill to drill 1-1/2"" holes through plywood using a flat wood boring bit. As he was drilling, the bit bound up in the wood causing the drill's handle to twist, bending his right ring & pinky fingers. (NOTE: Drill was NOT equipped with a trigger lock.) He let go of the trigger as soon as he noticed it bind, but couldn't get his hand away fast enough.
Ee was fixing a vent bag using a scissor deck. Using lever to raise scissor deck, something caught on the lever causing it to keep raising, thumb got pinched. Seen at the ER and wound closed with stitches. Released to restricted duty.
EE used the fork lift to place a steel wear plate on an excavator bucket to be welded. The plate of steel was placed on the lip of the bucket and needed to be adjusted into place. While pushing into place by hand EE pinched his right thumb causing a laceration requiring medical attention.
As the EE was tighting a plow bolt on a quarry loader his L hand came into contact with the bolt head and the impact wrench caused it to spin and lacerate his L palm.
Employee was driving a lube truck on a haul road. He went through a soft spot in the road, which bent the tie rod and damaged the oil pan. The steering wheel turned quickly, the employee's little finger got caught in the steering wheel as it turned. The employee fractured the 5th metacarpal on the L hand. Employee is required to wear a splint and a sling until healed.
WHILE INSTALLING SCREEN, EMPLOYEE LOST HIS FOOTING AND TWISTED HIS LOWER BACK.
