Mine site received approx 6"" of rain in less than 3hr period. Mine lost power twice allowing surface sump to be overrun with rain water. Rain water then ran into the main return, interrupting mine ventilation. No injuries.
Roof fall was found when EE and MSHA was making air ways at break #81 between #4 and #5 entry in north west mains
Unintentional fall of the mine roof was discovered at #10 crosscut, #5 entry of the Z-right section. Fall was approx 36'l x 18'w x 5' h. Fall was immediately reported by phone to the regulatory agencies and the crew was withdrew from the section. The primary life-line was re-routed and proper procedures were followed before any one was allowed inby the area.
Fall of roof occurred at the 7-25 cross-cut, track to return entry, at the Whittaker Portal. The fall is limited to the cross-cut only. Approximate size of the fall is 16 feet wide, 10 feet high and 30 feet long.
A fall of roof occurred at spad location 338+00 on the old main line track at the Whittaker Portal. The inby corner on the wire side fell out and pulled additional material out of the cross-cut with it. The approximate size of the roof fall is 8 feet high, 8 feet wide and 15 feet long.
The elevator in #6 shaft was inoperable due to a broken cable on the door. The elevator was out of service from 1:30 a.m. until 4:10 a.m.
A reportable roof fall occurred at F-Seam, #71 XC, between entries 5 and 4. The fall was approximately 17' wide, by 20' to 25' long, by 6.5' to 7' high.
An unplanned roof fall occurred at x-cut # 37 on 6 south belt. The fall was about 20 foot long, 20 foot wide, and 10 to 12 foot high. The fall will be cleaned up and rebolted.
An unintentional roof fall occurred on Unit #1 in Entry 5 @ 37+30. The fall occurred due to the strata broke in the center of the intersection above the anchor point. Several roof bolts were left hanging in the roof. The fall is approximately 0 - 5.5 ft. high, 18 ft. wide, 18 ft. - 20 ft. long. The fall will not be cleaned up.
An unintentional roof fall occurred on Unit #1 in Entry 8 @ 38+50.The fall was found by a mine examiner. The fall occurred due to roof slips coming together in the intersection. The fall is approximately 30 ft. long, 18 ft. wide, 6-7 ft. high. The fall will not be cleaned up.
A non-injury ignition occurred at approximately 1:15 pm during cutting-repair operations on the face conveyor (panline) on the 19A at #222 shield.

A call was made to MSHA HOTLINE in error that the Enlow hoist was down. What happen was the belts running over top the hoist house had structure break loose and the hoist was shut down as a precaution.
An unplanned roof fall occurred at 7:00 P.M. ,CST, at cross cut #47 on the first main west, number 5 entry. The roof fall was approximately 8 foot high and 15 foot long. The openings of the intersection where the fall occurred were approximately 18 feet wide.
The Crabapple elevator was inoperable with 25 persons on board for more than 30 min. No injuries
Roof fall #4 entry 60' inby spad #448. Dangered off and cribbed, move section outby for a distance of 600'
Towards the end of second shift the hoist (Koepe)final stop switch failed. This shut down the hoist creating an evacuation due to not having two escape ways to surface. The mine was evacuated with no injuries or other problems.
A roof fall was discovered above the anchorage in the 17-06 crosscut belt to track. No injuries, ventilation and travel are not affected. The fall is approximately 20' long x 7' high x 16' wide.
At approximately 1:30 am, a dozer operator reported observing pieces of debris raveling off the East highwall in the East pit. A supervisor observed over the next 90 minutes, portions of the East wall experienced a gradual block slough. No injuries or equipment damage occurred as all personnel and equipment were clear of the area. 7000-1 is files under protest.
Outby foreman went to build a stopping in this area and located the fall
A roof fall was found by examiner doing weekly exam at c.c. 52 entry 00 in main west. Fall was in a 4 way intersection and was supported with 8' full resin pins. Travelway was re-routed and adjacent area was timbered off. There are no plans to clean up this fall.
6B Longwall encountered a gas well at #4 shield 30+30 spad. No injuries, no ignition.
A fall occurred at an intersection 200' inby the feeder (A9-9 Heading at #30 Crosscut). The fall area was bolted with 8' SuperTwist bolts and 8'-12' CC Cable bolts. No ventilation, equipment, or personnel were affected.
16A Section experienced a roof fall. The fall occurred at 21 cross-cut. Involving 1 xc to 2 xc and the #2 intersection. The fall was approximately 16 feet wide, 15 feet high and 140 feet long.
Mine hoisting system inoperative due to electrical problem. Electricians had problem corrected at 1:05 PM
Roof fall found in #6 entry off of #9 belt between spad # 1718 and #1887. The roof fall was approx. 1,250' long and 3'-12 high in places. No ventilation nor escape way was blocked. This mine has been idled and in non-producing status for approx. 7 months. There was no old workings above or below this fall.
An unintentional roof fall occurred in the 4A return at 18+00 in entry #4. The fall did not affect ventilation, passage of men or caused personnel injury. The fall measured 20 feet x 20 feet x 10-12 feet high. The area had been roof bolted with 6 foot fully grouted roof bolts.
Highwall failure. 7/23 4:30 pm - 5:30 pm small amounts of rock started to fall to the existing bench. 7/23 5:30 pm - 6:00 am on 7/24/12 large amounts of rock from highwall fell.
Fall was in entry #4 on Sliding Hill Mains at break number 117. It measures 60' long, 20' wide, and 6' thick. Roof support was 5' torque tension bolts
Three contractor employees were using the elevator travelling from the 4th floor down to the 1st floor. They were transporting a brick saw and a pallet jack. The elevator passed the second floor and jerked twice and stopped. The elevator operator used the radio inside the elevator to call for help. The Elevator Company was called and got the men out at approximately 12:40 PM.
Took the #1 Elevator out of Service to shorten the car hoist ropes. Shortened the ropes and replaced two buffer switches. Tested the car and returned to service at 12:30pm. This was scheduled maintenance. No one was trapped on the car at any time.
A roof fall was discovered in the East Mains, Entry 5, at crosscut 53. The area had previously been supported with a set of steel arches installed in an overcast area as a King and Queen style set. The roof fell in above these arches and blocked the roadway.
An unplanned roof fall occurred on the 6C Belt at break #20. Fall was approximately 80' long by 20' wide by 8' high.
Cameron elevator stopped 10 feet from top with 1 man aboard.
Fall occurred in 17-06 crosscut from track to neutral. Travel & ventilation are not effected. Additional roof support has been set on both sides of the fall. No injuries. Fall is approximately 7' high x 16' wide x 20' long.
A fall occurred one black inby the feeder in the #5 Heading, intersection at the #20 Block. The fall area was bolted with 8' Combo and 16' CC Cable Bolts. There was a roof bolting machine inby the fall area. The only ventilation that was affected was at the bolting machine. No personnel were affected by the fall.
Weekly examiner found an unintentional roof fall upon his examination at the #8 1/2 crosscut in the #3 entry of the 8 Haulage bypass. The fall measured approximately 40' x 18' x 8' high.
The Cameron elevator stopped 12 feet from the top with 10 men aboard.
Roof fall due to kettle bottoms, mud stone and water saturation.
An unintentional roof fall occurred on the Main West 2 Return, Entry 1 between Crosscuts 86 thru 87. The roof fall measured approximately 20' in length, 17' in width, and 4.5' in thickness. A 103 (k) Order (8446917), was issued July 18, 2012, by MSHA Inspector.
Slope hoist's DC motor had a bad brush and was replaced. Operator was made aware of the problem at 9:04AM and called in at 9:15AM to cover the 15 minute rule. Hoist was back into operation at 9:25AM which falls below the 30 minute time frame.
A roof fall was discovered outby spad 33190 during preshift exam. The roof fall meas. 19' Wide, 30' Long, and 9-10' in Height.
An unintentional roof fall occurred at this operation at 13:45 at X-cut #7 between #4 and #5 entries at the mouth of the Headgate #2 unit.
A roof fall approximately 20'x 16' by 6' high was found by mine examiner in the North SubMain in Entry 10 between crosscuts 4 and 5. No harm to personnel or equipment was experienced.
An unplanned roof fall occurred outby the idle #2 section on 7/16/2012. There were no injuries or equipment damage due to the fall and there were no escapeways blocked. The fall was in the return and an alternate airway has been established. This is an idle section and the equipment has been moved outby to the #16 tailpiece.
A roof fall occurred at xc16 on the 1st Main East off the Main North between entry 5 and entry 6. The fall is 11,100' outby the loading point. The fall measures 50'L x 18'W x 10'H. There were no injuries and ventilation or travel was not interrupted. The belt structure had to be removed temporarily to remove some of the fall material and install secondary support. Map attached.
The guide rope on the South Mains Intake Shaft Emergency Hoist was damaged. The decision was made to idle the mine until the rope could be repaired. The rope was repaired and regular mining operations have continued.
An unintentional roof fall occurred in the 5th north headings MMU 012 return @ 48+15 in entry #5. The area had been supported with 6 foot glue bolts. The fall measured approximately 19feet x 19feet x 8feet high. The fall may be cleaned up at a later date. It did not affect ventilation, cause personal injury or passage of men.
A rock fall was discovered in the Intersection behind the power center at #2 break in the #5 entry along the #2 belt the fall was estimated to be 5' thick X 18' wide X 20' long no injuries and no ventilation controls or equipment was affected. This fall will not be cleaned up and the area has been cribbed/timbered and dangered off.
An unplanned roof fall occurred in Roadfork #51 mine, near spad #2399 that measured approximately 35'long x 20'wide x 8-12'high. No injuries were reported. The area was developed on 6/11/12.
The slope car at Roadfork #51 mine malfunctioned for a period of time that exceeded thirty minutes. When checked it was found that a hall switch had malfunctioned that affected the braking system. No injuries were reported.
A fire was discovered in the tall grass above the 1North highwall at 2PM. The grass fire was not extinguished within 30 minutes. A motor grader and mine fire fighting equipment had the fire contained and out shortly after the 30 minute timeframe. At no time were miners or mine equipment and structures at risk of injury.
Rock fall occurred on 1 North East Mains Brk #12 on the belt line. Fall measured approximately 40' long X 15' wide X 3' thick.
An unintentional roof fall occurred at the New Future Mine 3W Head gate belt entry crosscut 14. The fall was approximately 42 feet in length, approximately 18 feet in width and approximately 10 - 12 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up. Fall #29B
Roof fall occured in the 2 North Mains at crosscut 4 in the return air-course.
The elevator car would not leave the top landing. The brakes would pick up (release), but the car would not lower. Car drifted up and sat on CW buffers. Shindler was called. MSHA was called at 10:28. Found some dirt in the contactor relay. Cleaned and tested and returned to service at 2:19pm.
We discovered a smoldering portion of out coal stock pile. The smoldering portion of the coal stock pile was spread out and water applied to it.
While operator was conducting a shaft inspection from the hoist inspection deck, a tree struck the surface power line causing an underground power outage. The person performing the shaft inspection was unable to leave the hoist until power was restored approximately 45 minutes later.
Roof fall occurred on the 16-Butt MMU-003 section after deteriorating roof conditions were discovered. A attempt was being made to installed additional roof support but failed to prevent the fall. MSHA Hot Line was immediately contacted and notified. No injuries or equipment damage was reported as a result of this event.
Limit switch failure. Phoned MSHA @ 8:10 a.m. Trapped personnel were Maintenance Manager, MTM and Laborer. Time of release 8:30 a.m. There were 270 hourly employees, 43 salaried employees and 78 contractors working on site at time of entrapment.
Roof fall old room set up in #4 entry one crosscut outby spad #415. Roof fall for about 10' long and fell out from nothing to about 5 or 6' high. The area was double rowed posted and dangered off all access.
Coal was smoldering and was spotted at approximately 2:30 AM. Steps were taken to extinguish the smoldering coal. The smoldering coal was extinguished at approximately 5:50 AM.
A noise was reported in the #1 elevator. The compensating chain was contacting the floor. While attempting to correct the problem, the bracket that attaches the compensating chain broke. Clevises were used to replace the bracket (break away) until a new break away could be made. The car was returned to service at 3:33pm.
Crusher elevator stalled at 8:50 p.m. - MSHA was notified at 9:19 p.m. Employee was entrapped and released at 1:13 a.m. Load was offset causing the emergency brakes to activate. Seventy-seven employees were working.
During his weekly examination the fireboss identified a roof fall in the old 1st Rt. panel two breaks outby spad#986 in the #2 entry.The fall was approx. 20'W X 100'L X 4'-6'H it was bolted with 4' fully grouted resin bolts and supplemental 5' torque tension.The fall will not be cleaned up and the area has been dangered off and timbers set.
A roof fall occurred in the outby area of 4 Mains 40 feet inby survey station 210. The fall was approximately 40 feet long x 18 feet wide x 7 feet high.
The skip contacted the 1200 Level landing and caused the hoistman to shut the hoist down and dispatch personnel to investigate. The skip was twisted in the compartment as the bottom 100' of west guides had broken free and fallen down the shaft. The guide bracket failures were likely due to an incident that occurred the previous Friday, but the damage was not visible when inspected.
roof fall was found by m.s.h.a outby survey station 1593 in the primary escape way for the 001 mmu could not see the distance the height of the fall appears to be above the anchorage zone.
A roof fall was discovered 7/10/2012 at approximately 10:30am by the mine foreman. The fall is located in the A Right Panel in the right return at survey spad #'s 669, 670, and 671. These locations are in the number 5, 6, and 7 entries.
A roof fall occurred in the travelway to ventilation check point O. The fall was approx. 20' long, 15' wide, and 6-8' high.
WHILE PRE-SHIFTING #1 UNIT' EXAMINER FOUND ROCK FALL IN AREA OF 9+40 INTAKE AIR COURSE.
At around 10:30 am on July 9, 2012 the Schindler Elevator became in-operable. The door switch bracket on the door was broken. Repairs began immediately and the elevator was back in full service in 90 min. The failure did not entrap or cause injury to persons.
During a daily inspection an EE found one roller guide assembly was off the guide rail. Elevator was removed from service at 2:45am. Shindler was notified. MSHA & DEP were notified. Shindler EE arrived & assisted our EE in correcting the problem. The DBG of the guide rails was measured from top to bottom of the elevator. The car was returned to service at 6:50am.
Inspector was inspecting the #1 Elevator and noticed a guide roller wearing uneven. (The roller was installed 2 hrs. prior). Two workers attempted to adjust the guide, but the adjustmen bolt broke. Shindler had a spare bolt. The spare bolt was installed and adjusted. The guide rollertested ok and the car was returned to service at 5:40pm.
A roof fall occurred in the break between #6 and #7 entries at #22 XC in the Cedar Grove Mains. The size of the fall is approximately 60 long, 18 feet wide, and 5 feet high.
an unintentional roof fall was discovered at approximately 6:10 am in number 6 entry on the 3 east mains at break No 77. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up. The fall was approximately 11 feet high, 30 feet long, and 18 feet wide. The fall was in the intersection.
A lightning strike knocked out the power on the Camp Creek portal side. The service hoist would operate and the #7 and #8 fans went to diesel backup. Power was restored at 6:40am with no damage to equipment.
The old metal weigh hopper floor was attached to the skip hoist and was being pulled up out of the mine when the metal floor got wedged inside the shaft preventing the 7 employees at that current time to exit the crusher. MSHA was notified at 5:00 p.m. and the employees were hoisted out of the crusher at 8:50 p.m.
Roof fall at West Mains track, #5 Entry at #146 break. Rock measured 20' X 20' X 9'. The location was mined in JUL 1989 and was bolted with 4' fully grouted bolts and 6' cable bolts. The fall occurred 36,260' from the face.
During the preshift examination of all underground workings after a 7 day power outage a roof fall was discovered in the 1 South Mains track entry from break 28 to 29. The fall was 5 ft thick and 95' long. The fall was reported at 9:15 a.m.
This 7000-1 is being submitted in response to citation #7116747 issued on 7/10/12. Following an investigation, the operator is not in agreement with MSHA that an immediately reportable methane ignition occurred. While preforming routine cutting / welding operations on the 19A tailgate shearer drum near shield #22, a small fire occurred which was immediately extinguished.
A roof fall occurred in the return air course, N07 entry, one crosscut outby survey spad 193, above the anchorage, which did not affect ventilation or block passage of men.
While traveling the secondary escapeway, an employee noticed that a roof fall had occurred in the intersection measuring approximately 18' wide by 12' long by 7' high. 7' torque tension bolts and trusses were the primary roof support in the fall area.
During the weekly inspection of the secondary hoist a hydraulic valve, which supplies hydraulics for the raise and lower of the hoist became stuck rendering the hoist inoperable. The valve was repaired and the hoist was returned to operation.
Fall was found in Alloy #5 entry, at break 15, and 16. dimensions were 20'wide X 45' Long X 4' high. Roof support was 4' glue bolts.
Employee was busting rock with the excavator. he turned around to get out of the hammer and saw smoke. He got out with fire extinguisher, heard a pop and saw flames. He stood on the tracks and saw flames in the engine compartment. He tried to put it out with the fire extinguisher but could not so he got on the ground to fight the fire.
We were taking a 10' cut by 16' wide out of #5 heading where we were going under a hollow and as we were cleaning up the cut I saw some of the top drip from the rib and we stopped and backed miner out and was going to set some cribs but in about 10 mins. it started to fall from the face out.
A roof fall occurred in the #2 entry of the Sturgis main in the return at spad number 93+68. The fall was approx. 50ft by 18.5ft by 6 plus ft high. This fall is approx. 770 ft from the nearest section
A roof fall occurred in the #5 entry in Main A one crosscut inby station number 167. The fall was approximately 20 feet long x 20 feet wide x 5 feet high.
A roof fall was discovered in Flat Creek Main return. It was approx 10' x 8' x 15'. Some of it was above the anchorage point.
A roof fall occurred in the 7A return air course at station number 1655. The fall was approximately 25 feet long x 18 feet wide x 7 feet high.
A rock fall occurred in 2nd Southeast Sub main in #7 entry at 5+80 in intersection. It measured 50ft.long x 19ft.wide x 8ft.thick.This fall will be loaded out bolted and cribbed.
Roof Fall occurred 7/2/12 at section 003 at spad # 11550. 1000' feet from active face. No personal injuries, No Equipment involved. No ventilation or passageways affected. 220'L. x 18' W. x 8-10' H. Entrances have been breakered off.Mining under 300' cover.
While lowering men into shaft, a work deck bumped a steel bunton, damaging hoist rope. Men were evacuated using secondary hoisting system.
The brake coil for the 8 Haulage elevator overheated, and had to be replaced. The elevator was inoperative from 0910 until 1100 hrs. This elevator is the secondary escape hoist.
At 6:23 PM on 7/1/12 the site lost power shutting down the fuel system. The system was restarted at 8:55 PM on 7/1/12. At 8:55 PM the coal mill experienced an unplanned ignition of the coal in the #1 KFS Ball Mill. No injuries or property damages occurred.
The break coil for the 8 Haulage elevator shaft became overheated. This is a secondary escape way.
While the injured was attempting to change the batteries on the # 8 scoop, the # 11 scoop was traveling outby and impacted the # 8 scoop causing it to move and pin the victim against the coal rib.
At approximately 4:00am on the 27th of July 2012 a fatal incident occurred when the boom of the continuous miner struck the employee. The accident remains under investigation.
Full details are not yet known. Employee was lubricating a conveyor and the conveyor was started. Injuries are either from being thrown from the conveyor or being trapped in machinery.
Witness saw the water truck traveling downhill on Ridge road & then saw a cloud of dust. Truck traveled downhill(9% grade)struck berm on the right side of road ejected operator changed direction & traveled across road striking the berm/slope on the left side of the road. The truck overturned & came to rest. The victim did not appear to use his seat belt. Minor damage to truck.
IE and another EE were adjusting the tracking of a conveyor belt. After completing that task, IE went inside the guard and reached into the conveyor belt to clean out material. A roller grabbed the pliers he was using and pulled his hand and arm inside. The other EE hit the E-stop, but the hand and arm was severely damaged.
Employee was lifting the step of the track mucker to put it into the mucker bucket. Employee was assisting the lift with his leg and his right little finger was caught between the step and the mucker bucket. Right little finger was fractured just below the first joint.
Employee was scooping #4 Hdg. and curtain fell down. Reached up to get curtain and finger got between canopy and roof. Cutting off end of finger.
Individual complained of pain in his side he felt that it was a possibly a hernia. Individual went to doctor and confirmed same.
Employee was exiting the operators deck when his feet hit the tram pedal and lever, sliding the operators canopy of the roof bolter into rib. Employee's fingers were between canopy and rib resulting in right hand index finger to be cut off.
The employee was dismounting the D-75 Cat dozer via the right side push arm. The employee had inadvertently stepped on a rock with his left foot, at which time he fell to the ground and felt pain in his left knee. Employee stated he maintained 3 points of contact while dismounting.
Employee was mucking in a cabbed mucker when he hooked a boulder with the bucket & it slipped off the boulder causing him to hit his head on the inside of the cab, Resulting in neck pain. Went to medic on site. We took to ER. X-rays were negaitve.Doctor issued restrictions. He missed 2 days and received a Full Release back to work.
GRINDING DISC BEING USED BY ee SHATTERED CAUSING PIECES OF DISC TO FLY BACK HITTING ee. PIECE OF DISC BECAME LODGED IN ee RIGHT SIDE JUST BELOW RIBCAGE.
Was tossing crib blocking thru man door when his hand got pinched between door frame and block he was tossing.
Employee was working on grader when his foot got caught between the pedals. When he went to sit down, his right knee got twisted, causing pain to the knee.
EE was pushing a roll around ladder into position to open the west cleaning caustic chain valve when he felt a pop in his right calf. Muscle strain.
Employee was changing fuel filters on a D10R dozer and while putting the side door back on the employee felt a stinging sensation in his neck. Note: Employee did not miss first day of work until 8/11/12
Going back down ladder after rain to ledge #1. Foot slipped and bounced down ladder to bottom 8-10 foot. Broke left arm between wrist and elbow. Bruised right hip.
Employee was using a sledge hammer to remove a solid block stopping in 14th east return, when he felt a sharp pain in his lower back. He reported the incident and 5 days later (8-5-12) he went to the doctor who kept him off from wok for 4 days.
Twisted leg getting off dragline. Pulled arm to avoid fall. Accident occurred 7/31/12. Off work since 8/14/12.
While attempting to lift the Tetmite drill head, employee felt a ""pop"" in his back.
Ask employee to go to storage shed to get new screen for our wash plant, upon doing so the screens fell and cut his leg
Scraper coming out of waste area lunged to the side hitting another scraper stopped to the side of the waste area, causing the operator to fall to the ground.
Employee was energizing an airline, he turned on the wrong valve and that airline whipped off the hooks and hit him in the left lower leg causing a tib-fib fracture.
Employee was welding pin back in place on left side of bucket and a hot spark went into left ear.
EE was working on clamps on Pioneer belt. A sliver of metal entered eye. EE was wearing glasses and face shield.
Employee was barring over a relay bar when the bar slipped off an came back and landed on his left ankle. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
Employee was tightening a pressurized hydraulic fitting to stop a drip leak. He had barely touched his wrench to the fitting when it gave way releasing pressurized fluid. The crusher was NOT on/powerized but has contant pressure in the valves where the fitting was. The pressurized hyd. fluid shot out and cut his arm and neck.
Back sprain caused by repetitive use of an 8 pound sledge hammer.
Employee was operating the #10 shuttle car, as he stepped out of the car to turn his body around he twisted his knee. Mine management was not aware of this incident until 07/31/2012.
EE was operating a roof bolter on the operators side when a piece of the rib measuring approx. 75"" l. X 11"" w. x 4"" t. rolled out and landed on his left ankle.
Employee was walking in #1 entry on #3 unit when he twisted his right knee.
EE was installing a chain flight in wash box while he was lining up flight he got his right thumb caught between the flight and chain sprocket in sprained his thumb.
EE was getting on a dozer when he twisted his right ankle.
Employee was swinging his drill pot out to install a bolt and had his hand resting on the supply tray of the bolter and his finger was mashed between a bolt and the side of the tray. He was taken to ER and received stitches to close the wound and a splint was applied.
Employee was lifting about ten lbs. While setting it into a kubota RTV she felt pain in the lower back. The pain worsened through the night. She saw a physician the next day. She was diagnosed with muscle strains in the lower back.
While bolting the right rib, the steel slipped out of the wire hitting his hand that was on the machine. His hand was hit by the drill steel. Went to medic on site. We took him to ER for x-rays, which resulted in fracture to index finger on left hand. Restrictions were issued.
A haulage truck driver was returning to a dump loaded, when he lost control of the haul truck, hit the berm and tipped the haul truck on its side. As a result of the incident the operator sustained injuries to his torso.
Employee was drilling & in the process of pulling the jackleg from the hole, a small rock from above struck his right arm, causing a contusion. He went to medic on site. We took him to ER for x-rays, which were negative. Doctor did issue restrictions.
EE was plugging a motor in a battery and the cable shorted out.
Employee was using oxyacetylene torch to cut a metal pipe when spark blew back in his right ear. Employee sought additional medical treatment on Sept. 22, 2012 and began missing days from work on 09/23/2012.
Employee states he was erecting a boom on a crane & fell on to the crane track fracturing his R wrist, damaging ligaments in his L knee and spraining his L ankle. The employee didn't seek medical treatment until 8/5/12 & this office was not notified of the incident until 8/8/12 when the employee was taken off work by the treating physician. This is a lost time injury.
Employee was cleaning spillage around tail pulley at plant with water hose. As he walked to the water truck and stand to increase water pressure, he was struck in the left shoulder/back area with an unknown object. The impact resulted in pneumothorax (collapsed left lung).
Employee was shoveling tailings into drag chain on the Mucker, when he lost his balance and his left foot went into the Mucker.
EE was walking beside power box when a piece of rock fell out between bolts and hit him on his back. Strained his back and bruises.
In preparation for painting a new conveyor, one end was resting on a cross-tie and the other end was raised by placing fork lift forks under a tail pulley. As the conveyor was raised to install blocking, the conveyor rolled off the forks striking the left ankle of the individual, fracturing his ankle.
Employee alleges he was reaching up to put a roller under the conveyor belt and he felt pain on the right side of his groin area. Causing a strain to his groin.
The employee opening an airlock door felt burning and pulling in his lower back. ""As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV.
Checking levels in oil tank step down off tank pad (apporx 14 inches) and twisted knee
While walking along side a parked shuttle car, the employee slipped on some loose material causing him to fall onto his left hip. The employee continued to work. The incident has now become reportable based upon medical evidence received on 8-22-2012.
Employee was bent over picking up line curtain. When he lifted up he was struck by a rock that fell from the roof. The rock hit him in the upper back across the shoulder blades.
Employee was stepping over a ditch that had been dug to drain water from a work site. The ground was wet and slick from the rain. When he stepped over the ditch and his foot landed on the other side, he slipped and hit his arm on a rock hurting his shoulder area.
Employee was in the process of cleaning a tail piece. As he was shoveling under the tail piece the bottom belt came in contact with the shovel, jerking the shovel causing pain in his right arm. He ceased work and was taken to a local hospital where X-Rays were taken. He was taken off work until a follow up on 8-2-2012.
Employee was putting ANFO into the ANFO Pot on explosives rig. He picked up a bag of ANFO (50 Lbs.) and when he did so felt something pull on his lower right abdomen side. Doctor believes that it is a hernia.
Two employees were in a pump truck that parked at the top of the D3 #1 ramp. One of the employees got out of the truck. While outside he dropped his snuff can and when he reached down to pick it up a water moccasin bit his left middle finger.
After bending over to measure grating, injured EE stood up and felt lightheaded and dizzy. Employee fainted, collapsed, and struck the knockout drain frame on the log washer. Bruising was apparent. EMT was contacted. EE was transported to the hospital for X-Rays.
Employee was changing oil in raw coal viberator when he inadvertently stepped back off discharge end.
Employee was cleaning curb ring a rock fell and cut his right index finger.
Employee was cleaning out his bolter steel that was stopped up by beating it against the bolter head and caught his finger between the steel and the bolter head pinching it.
He slipped on wet ground as he was getting out of Bobcat and landed on back. Bobcat was parked on concrete slab and he dismounted properly but foot slid on slick of mud.
Climbing down track to check for a leak, and foot slipped off pad. He held on with both hands but groin area landed on track
The employee hanging miner cable by himself stated he felt a burning sensation from the middle back up to including his neck. ""As requested by MSHA, this information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV"". 8/7/2012 this claim was denied (no medical evidence).
EE was assisting changing belt rubber out(265'). The belt was clamped off it had been cut and hooked to the scoop. EE released belt clamp after the comealongs had been released. After clamp was removed the weight of the belt caused belt slack to release and caught EE between the belt and the jack.
Employee was tightening bolt at #3 screen (rear rubber flap), wrench slipped off head of bolt and caused strain on employee's mid-section (right side just above belt). Employee was not struck by wrench or bolt.
The employee was in the # 3 entry rolling up a ventilation curtain in preparation to scoop and dust the entry when a piece of draw rock fell from the roof striking the employee on the back.

Employee was running the bobcat and when he was getting back in the seat he hit his head on the front of the equipment jamming his neck.
While in a kneeling position, cleaning out the deck of the shuttle car, a piece of draw rock fell from the top hitting employee on the ankle.
EE was carrying an F-Bar chain from end of 16A track, when he went to drop it the hook caught on his mine belt causing him to strain his back. After being evaluated by a doctor he started missing work on 8/6/12.
The employee was drilling holes into the rib so rib bolts could be installed, when the steel struck him on the right forearm
Employee was installing rib bolts when he stepped back and got foot caught between cinder blocks causing knee to twist. Location was MMU 001 60' outby SS #377
Employee was cleaning cab of scoop out when a rock measuring 16""x24""x8"" hit him in the right side of his rib cage.
Employee was inby utility on # 2 unit, after walking unit fixing ventilation controls he sat down against the rib in # 11 entry and a rock fell from area above head striking on left knee.
Employee had moved the miner into #5 entry on #3 unit and while pulling on the miner cable he strained his back.
He was coming down the hill at Alloy between the 6 and 5 marker and tried shifting gears then lost control of the truck and rolled over around marker number 4.
Section foreman was helping install a belt conveyor drive in the belt entry. At which time EE was attempting to move a bearing on hold down roller. By striking it with a hammer while striking it with another hammer. I small particle of metal dislodged from one of the hammers striking him in the right eye.
Employee was lifting a Korfman chain saw with a hydraulic jack to level it for a cut. He had the jack set in 2 inches of mud, and the jack tilted to one side when it took on the weight of the saw. The machine came toward the wall and pinned him at the knee. Injured left knee, chipped small piece of bone on patella, bruised knee area.
On 7/25 EE said he was going to chiro. He did not say he got hurt during work. Not convinced this is work related injury because of what ee did over weekend on July 28, but found out on 8/22 that work. comp. insurance will be treating injury as work related until further investigation.
Roof bolter helper (injured employee) was pulling slack cable off the roof bolter cable reel. The roof bolter operator accidentally started to wind the cable reel while the bolter helper was pulling it, causing the cable to jerk the bolter helper and strain his back.
This injury occurred to a driver while on mine property at the Tilden Flux dump site. As the driver was pulling his wheel chocks from underneath the tractor tires in preparation to leave the dump zone, the driver stumbled backwards and stepped on his left foot at an odd angle. After receiving treatment at a local walk in clinic, the driver diagnosed with a broken bone in his foot.
Employee was cleaning the area in the dog leg of 15th East in preparations of a seal to be installed. Employee attempted to move a rather large piece of rock from the area when he felt a sharp pain in his lower back. He finished his shift and 5 days later, on 7/30/2012 he claimed to not be getting better and asked to see a doctor.
After hanging the miner cable around the intersection in LOB #6 entry, EE was talking to the foreman. The scoop operator inadvertently hooked the miner cable 2 breaks away. Causing the cable to fall down and drag across the intersection causing ee to trip over cable. Resulting in falling on a baseball size rock bruising his back.
EE was shoveling under the primary tail pulley conveyor after the plant was shut down and he developed pain in his lower back. This was his normal job. The pain was bad enough that ee had it checked out at the emergency room and they said to rest for 5 days.
Riding in a battery powered mantrip,at a belt drive boom hole, the mantrip driver decided to turn the mantrip around,the victim did not keep his head down in the low seam height and he hit his head on the mine roof.
While loading roof bolt plates onto drill in walkway, EE twisted to pick up plates and felt a pop in his back.
EE was greasing feeder. When he walked around the fire boxes and the section tailpiece, he tripped over the hydraulic drill and hoses.
The employee was helping pull rail out of a cross cut into the track entry, the crew was using a manbus to pull the rail, the pull rope that was being used to pull the rail broke causing the rail to swing and hit the employee in both legs.
Employee was performing cleanup in the shovel pit when he hit a bump and his seat bottomed out. his lower back began to hurt and he called his supervisor. He was taken to the ER and taken off work until 7/27/12.
Employee was using his foot to push a railcar unloading door closed when the door swung back and struck his foot, causing a ""pop"" in his knee. Initial Dr visit indicated bruising and employee returned to work w/no restrictions. This accident did not become a LTA until 08/14/2012 after a follow-up visit to the DR.
Employee stated that while exiting the cab of the dozer he lost his grip falling.
Employee stated that while walking across the section, he slipped and twisted his right knee. Employee continued to work until 11/08/2012 at which time he began missing work.
A small rock 2""x3"" flew off the drill pot and hit employee's safety glasses.
On 7/24/2012 at about 11:45 pm an employee and injured employee were replacing idlers in the 5A belt. IE was pulling on the old idler to free it when it came loose all at once and fell and hit IE on his right foot.

Employee was walking when he struck his head against a bent roof bolt.
Employee was picking up a heavy battery with his right arm, (the battery was stuck). His arm popped and crackled when he first lifted it out of the battery box. He said it didn't feel right and it hurt immediately.
Another mechanic using sledge hammer when it slipped out his hand and hit ee in face causing lacerations and contusions. On 11-21, after a CT scan, dx with a fracture of the nose
Employee was descending the steps on the HD325 fuel truck and he slipped and caught himself with his right arm straining his right shoulder and neck. Employee completed his shift and went to Williamson Memorial Hospital and was taken off work for a possible pulled muscle.
EE allegedly claims right shoulder injury from losing control of a fuel hose on Lube Truck #77 on 7/23/12. He stated the hose jerked out of his hand when he was cleaning it. EE did not report injury until 9/14/12. Workers' Comp denied claim on 9/26/12.
Employee had his right hand on the batteries of the 488 scoop when a piece of cap coal fell from the mine roof and struck him on the right thumb.
Employee was working on the feeder; slipped and fell and felt pain in back. I was informed of the incident on 8/7/12
Injured was hanging a shuttle car cable and felt a pain in his left bicep. Did not miss any work until 09/24/2012.
While removing steel out of RC4 belt, he mashed his hand on a roller.
Injured was walking across dredge deck to turn on lights, tripped over the hoist cable, fell to the deck and injured his left arm. The original diagnosis by attending physician is a broken left elbow.
He was splitting a block of flagstone and it had a crack which he didn't see it broke off and injured his leg.
Helping install pipe to drain water in pit. Stepped up on pipe and right knee gave away.
Employee was operating a continuous miner. He was cutting in the #5 entry face. He backed the continuous miner up to set over for his second lift cut. He reached down to pick up some supplies that had fallen off the continuous miner and a rock slid off the continuous miner striking him on the right ankle causing a fracture. He began missing time on 7/25/2012.
Employee was riding in scoop when it hit a rock in the roadway.
EE was pulling in an upward motion trying to loosen a flange on top of the cement silo. ***EE missed no work due to this incident until he had rotator cuff surgery. First day work missed was 4/18/13.
EE was opening up a valve when he felt a pain in his right groin area. On 07/24/12, EE diagnosed with a inguinal hernia. EE scheduled to have surgery, first day missed was 07/24/2012.
The injured employee, a supervisor, states he was lifting a bucket tooth for a front-end loader when felt pain in his left shoulder. He continued to work, but went off work for a surgical repair of his shoulder on 08/14/2012.
While hanging high voltage cable a section of the cable started to drop out of HV hanger and employee attempted to catch the cable. He felt a pain in his right shoulder. The employee stayed on the job until 4/17/2013 when his doctor took a MRI of his shoulder.
EE was roof bolting he was installing a cable bolt. It became stuck and turned the rotation on and the bolt struck his left wrist.
Another EE was running scoop unloading a new scoop bucket pulled bucket on the ground. EE pulled up to slack the chain, IE stumbled in front of scoop that EE was running mashing IE leg in between the buckets.
Employee was advancing discharge line for a face pump. While pulling it he twisted wrong and pulled his back out of place. Employee continued to work until 7/25/12 when he reported the accident and sought medical attention.
While walking down stairs employee misjudged the distance of the last step and fell. Employee has an Inguinal Hernia. Medical evidence shows this may be a re-occurrence of a non work related previous injury.
While setting grizzly grating at DB-01, employee's right hand was struck by grating and caught between grating and concrete wall at basin.

On 7/20/2012 employee was in the process of cutting down some old mesh. While cutting down the mesh he was struck in the head with a loose piece of top. He was seen by a Dr. on 7/27/2012 and was released to work that day. He continued to work while waiting for industrial to accept his claim up until his surgery date of 5/6/2014.
While removing a bit from a sub with a hammer, a small piece of metal was lodged into employee's right arm. Doctor recommended leaving it, but employee got second opinion. Became lost time incident on 07/27/12 due to surgery to remove metal.
Employee was lifting a portable tool box when he felt a pain in his lower abdomen.
The employee was installing a roof bolt from the right side of the continuous mining machine. Employee was installing an additional bolt in the roof strap at an angle when the bolt wrench and the roof bolt head broke, causing the bolt wrench to strike the employee in the face below his nose. Employee suffered facial fractures and cuts requiring sutures.

The man was tramming the continuous mining machine from an entry into a crosscut when a rib rolled out and struck the man on the back.
Employee stated that he was pushing trees down with dozer. A tree dented engine compartment door. He was going to lay door off of dozer onto ground. Standing on tracks, pried on door, door came loose, employee lost balance and jumped onto ground. Hit left hand on push beam fracturing left ring finger.
Employee was walking back from putting up bolts to move roof bolter to next row when a piece of draw rock 3.5'x 3' x 2"" fell striking him on the back of the head and neck.
Stated: Employee bent over to hook up a chain to a belt drive. He felt pain in his lower back as he stood up. He continued working. He went to the doctor on 07/20/2012. He was treated with medication. He has history of back pain/injury. Will RTW when pain subsides.
Cleaning around the wash plant with walk-behind Bobcat. Walked backwards into crusher part beside crusher, pinning left leg between crusher part and Bobcat.
The injured stepped between the frame of the roof bolter and the man-up platform while supplying roof bolt plates. This resulted in an muscle strain to his lower back. The injured wasn't diagnosed until Wednesday, July 25th.
Employee set lunch box down to go load a skid steer loader on an equipment transport trailer. As the employee set his lunch box on the ground he heard two (2) popping sounds and felt extreme pain in right shoulder.
Employee was conducting maintenance work with three others on a shovel. They were removing the upper rails on the shovel using an impact gun and wood skids. Later in the shift, the employee felt pain in his lower back. The employee did not report the pain until 8/2/12. He went to the hospital on his own and they found a disc diffusion in L4-L5.
Employee had degreased area around Conveyor #8 reducer. Employee was descending stairs when he slipped and twisted his right knee.
Employee reported being at mud lake #4 at 8:30 am on 7/18 when he was attempting to stand an 8 inch PVC pipe (Pipe weight= 50 lbs) upright, when his left foot slipped backwards, resulting in a pull in the left groin area. Result after examination by Dr. was a left inguinal hernia.
Employee was assisting with propel gearing work during an annual outage. When pulling an intermediate pinion the gear unexpectedly shifted and a hole on the side of the gear caught the steel toe of employee's boot bending his foot over the side of a platform he was standing on. The incident caused a fracture of employee's left foot.
Coupling hose from an emulsion truck tanker was being detached, the driver did not allow time for the air to release from the hose when he uncoupled the connection thus being under pressure the coupling struck the driver in the side of the head, as a result the driver sustained a concussion.
EE was called on the company radio and told that the Jaw Rock Chute to the ground was plugged with a big rock. He walked up the small pile of rock under the chute in order to dislodge the large rock. When the rock was dislodged the other rocks in the chute began to flow and he turned to see what the noise was. At that point he felt a pain in his left leg.
While jacking a flat car employee caught hand between Jack and car.
While loading some junk tires, the injured EE twisted wrong and hurt his back.
Walking back from roof bolter came through a fly pad rock fell from top striking him on head and right shoulder.
Employee was turning around in shuttle car to head back to the miner after dumping his shuttle car at the feeder. When he was turning around he twisted his left knee.
A 2"" aqua mine water line fell striking employee on head and neck.
Employee had connected a quick coupling on the scoop when the scoop operator engaged the PTO, the recently coupled hose became loose and struck the injured employee in the chest.
Employee was getting out of his truck before his shift started. He stepped out of the truck onto a small rock causing his left ankle to twist. The truck measured 36"" off of the ground.
Injured had been setting timbers along rib line at the 170XC of the Main South 3. The crew had moved a rock away from the rib line in order to install a timber, injured took a step backwards and stepped awkwardly on the rock. This caused him to lose his balance and twist his left ankle. X-rays revealed the ankle was fractured and injured is now off work.
Maintenance employee received a sore left neck muscle from hitting his head on a catwalk structure support during the lubing and cleaning of a screen.
Employee stepped out of tool trailer and twisted his foot on the large rocks that were used as back fill. He fell and hit his head on a tamper. He continued to work after the accident. Then on 7/26/12 he didn't return to work for his next shift. He is now claiming he is having headaches.
Employee was walking around a pillar to get a tool and trip over a rock and fracture his elbow.
Employee reported his injury two days after the occurrence to the personnel department. He stated that he was helping to install a pump when he hurt his back.
While hooking up a trailer to a lo-trac the employee asked the forks be raised. When the forks were raised, one fork came up quickly and came back down pinching the inside of the right leg between the fork and trailer causing a contusion. It was later determined the employee suffered an acl tear which required surgery. He began missing work on 8-22-2012
Carter bin was connected to a D-8 dozer. Pin was pulled to release the dozer and the bin began to roll back. Employee attempted to stop the bin causing injury to right leg.
Employee was using a sledge hammer to break a nut loose on a crusher. He complained of back soreness the next day. He did not note having any pain until he was at home that night.
Employee stepped off ladder into water. The water went into his boot, getting his foot wet, which resulted in a lime burn.
Mine Foreman was backing up his 3 wheeler ride when the miner cable caught the back of his head bent into chest causing neck and left shoulder pain. NOTE: EE did not miss work or seek medical attention until 7.23.12
EE stated he was walking along track from rail ride to scoop when a piece of rock fell from top and struck him on his back.
EE was moving C.M. cable out of the haulage roadway when a piece of rock fell out from between bolts in the mine roof and struck him on top of his left arm causing a laceration that required staples to close. Returned to work but dirt inside the opening caused him to return to hospital where he has been taken off work at this time. This is a LTA.
Employee was attempting to put a roller back in when he strained his shoulder. Employee started missing 09-06-12.
Unloading tailgate drive with a Petito mule went to adjust canopy and jack supporting it failed causing the canopy to drop striking his head, neck, and shoulders. PPE was not a factor, compliance rules/regs was not a factor, mining/equipment systems was a factor, miner proficiency was not a factor.
Employee was pulling trailer pin with a 6' pry bar when pressing down with force felt pain in neck, arm & shoulder.
He was hand pulling cable slack when he felt pain in his left knee. *We dispute the allegations in Citation #8049155 and submit this report solely to terminate the citation.
EE was loading supplies into a scoop tractor bucket. His right hand struck the scoop tractor bucket.
Employee was trimming saplings off the river bank and while in the process of setting the chain saw down the chain saw struck the inside of left leg under knee resulting in laceration requiring 16 stitches.
Employee was putting bolts in the #6 tail pieces skirt and the belt started while he was on the belt. Started missing work 7/17/2012.
Employee was helping to unload the stage loader concave, while unloading it he was standing in the red zone and got pinned when the concave slid on the scoop duck bill and put him against the rib. PPE was not a factor, compliance rules/regs was a factor, mining equipment/system was not a factor, miner concentration was a factor.
Employee was moving a piece of stone and felt pain in his back. Incident was not reported until 7/17/2012.
A haul truck operator was backing under shovel bucket, when the bucket hit the bed of the truck causing pain to the driver's back.

Employee was stepping up to get into trackhoe, his foot slipped off of the step. Employee fell on his left ankle.
He was dragging a crib block behind him in a low, old area of mine. Crib block became stuck, he pulled on the rope to dislodge the crib block. When it popped free it struck him in the left ankle resulting in a sprain.
Employee was walking underground, he stepped on uneven mine floor twisting his knee. He began missing time on 8-24-12.
While getting out of his tractor, he stepped into a hole and twisted left knee. He continued to work full time without restrictions until he was scheduled for surgery on 11/07/2012.
EE was part of a crew that was docking a barge. While attempting to place the docking cable over the ballard, EE experienced pain in his lower left back. EE was treated by onsite medical staff and returned to work. On 07/16/12, EE was sent to external doctor for evaluation. EE taken of work by treating physician on 07/17/2012.
Employee stated his knee had pain while rushing to get to the man van after shift. This was not reported to this supervisor until the following morning. Employee sought treatment on his own prior to reporting.
EE was walking off belt line and stepped into a water hole and rolled his left ankle. EE went above ground at 8:00 am. He was taken by ambulance to Wheeling Hospital where he was checked out by a doctor and diagnosed with a high ankle sprain. Hospital gave him 3 days off/excused.
This is an aggravation of an old injury due to lifting on feeder flights. EE was loading flights on Saturday July 14th, when he woke up Sunday Morning July 15th his back was hurting, He went to the Dr. that treated the original injury on Monday the 16th. The Dr done massage therapy and released him back to work on the 17th.
Employee was hanging utilities & pipe. He was loosening a nut on a v-clamp & felt pain(discomfort) in his left shoulder. He went to medic onsite. We took him to Doctor. X-rays were negative. However, Doctor issued restrictions.
Employee stated while lifting spreader chain to hook to loader bucket, he felt pain in his right elbow.
Employee was building brattice when he was wedging top. He drew back hammer and felt pain in his shoulder.
Employee was getting out of the rear of a service truck when he had one foot on the bumper and the other on the rear step. He slipped & fell striking his head on a concrete pad used to service equipment.
EE was wearing protective knee pads that began rubbing blister on the right knee causing inflammation and redness. EE worked 3 hours on the 20th, could not finish shift, he is going to seek medical attention at Logan Regional.
Operator was cutting a crosscut. As mining advanced, operator stepped away from rib mesh to look at cutting. At this point a rib fell and knocked operator to the ground. Operator received two broken ribs, cuts to the face, and a pulled shoulder.
Employee was installing a spot bolt when a rock fell, struck the drill pot then bounced striking the employee in the mouth.
Operator slipped while dismounting and fell injuring his left shoulder
Employee stated that he was carrying a 5 gallon bucket of oil to dozer and felt a pain in his back. He went to the doctor a couple of times and was taken off of work on July 27, 2012.
He was replacing a piece of metal on front of shuttle car, he picked it up and turned to put it in place, when he felt his left knee pop. **(continued to work without restriction , until 8-31-12 when he went off for surgery)**
Employee had struck an equipment and was trying to re-install the door. The door fell hitting his left leg.

While installing liners EE's loose control of liner and it falls making contact with EE's right thumb resulting in the thumb being severed.
Employee was traveling underneath a belt line guard located on the 2nd Main Belt Line at crosscut 7+40 and stuck his head on the guard causing an injury to the back of his neck.
Employee was exiting the excavator, as he stepped down on the track he lost his balance due to the track being wet and twisted his left knee.
The employee was rib bolting on the 12A Section. A rock fell from the top, struck the continuous miner and then struck the employee's right foot. The rock fractured a bone in his right foot.

Employee felt discomfort in lower back after being loaded at the 17 Shovel. Evaluated at Gila Health Resources on 7/12/2012, classified as First Aid. On 7/17/2012 employee was re-evaluated by Gila Health Resources and incident was changed to Lost Time.
Bent over to lift a 6' timber to set it. Lifted timber twisted to set it and felt pain in his back.
Piece of belt covered with rock dust - while carry belt structure out of the belt line he didn't see the rock dust covered belt laying on the floor and slipped on it.
Employee was blocking the slope belt against movement with a belt block. He attempted to position the belt block and felt a pain in his shoulder.
The employee was replacing a belt on finlay screen when he stepped off the track on to a guard (on the ground) from the screen that had been removed for the belt replacement twisted his ankle fell and broke his leg.
Based on information received 11/30/2012. The employee claimed he fell off a ladder at the belt drive rock box and bruised his right arm pit. Medical received 12/03/2012 suggest no acute injuries. The employee has medical history on body parts.
Employee was moving (dragging across floor) a stool over to the threader when his back popped. This resulted in lower back pain.
Normal work station position sliding 80 lb cement bags on air table while palletizing bagged product. Worker stated he heard pop in shoulder and felt pain after incident in right shoulder.
Individual was pulling bolter cable at Main North Haulage 166 block when he felt a pop in the right shoulder. Individual continued to work until 7-18-12 when the doctor removed him from work.
Employee states that he struck his head on the underside of scoop canopy. Received medical documentation on 7/27/2012 of neck strain and physician gave employer two days off from work.
Employee stated he was lifting water line and his back gave out. Employee was taken to ER. Employee returned to work on 7/11/12 and continued to work until 7/17/12 except for his scheduled day off Sunday 7/15/12.
Slipped on truck step with one foot and other foot hung in step causing ankle sprain.
The employee was exiting the Cat 235 excavator and he alleges that as he stepped off the machine he stepped on a rock and fell to the ground
Employee was bent over picking up a cable and a piece of rock fell from the mine roof and hit him in the back and neck causing a contusion. He went to the hospital to have it checked and is scheduled to return to work on Monday July 16, 2012.
EE was walking around the long wall stage loader he was carrying parts. He slipped and sustained a groin pull.
Employee was standing out and in front of a 345 Trac hoe giving directions to the operator helping him guide the track back on that had jumped off the bottom rollers when a metal splinter came off and entered his arm causing a cut muscle.
While re-positioning continuous miner ee & another ee was untangling miner cable ee was pulling on the rope that is used to pull the miner cable. EE told other ee he hurt his back.
Employee claims he suffered a hernia from lifting and pulling a shaft. Did not report it until 09/27/2012.
While removing bowl assembly was nudging cylinder to make contact. Employee completed task 20 times, each time removing hand prior to pushing. He then rested hand on the rear of the cylinder. As it pushed, his finger was between it and another cylinder. It rotated pinching the end of the pinky finger. He lost the end 3/4"" tip of his finger.
EE was climbing onto side of bolting machine to install glue and bolts, when he pulled a muscle. Ended being very minor with no long term effect at all. He only missed 8 work days.
Customer driver was climbing a pallet in the bag house loading bay from ground level to concrete dock level when the pallet slid down and hit his elbow on the concrete.
Employee was removing a discharge line on a chemical pump when fluid came out and sprayed him in his left eye behind his safety glasses. His eyes were flushed out and he went to the doctor and was told to be off for 2 days.
Employee stated that while lifting high voltage cable and strained his lower back. Reportable lost time injury 9/28/2012 due to surgery.
Employee reported un-witnessed accident, stating he slipped on bathhouse floor, he finished shift and went to doctor next day, employee had MRI on 07/16/2012, no tears or deformities were found, however a cyst was found. Operator has determined no accident occurred, submitting for MSHA review, as Occupational Illness
Carrying pump down bank of pond, employee slipped and fell pump landing on his left side.
The employee tripped and fell striking his left knee against the section power center. The event resulted in a lost time injury to his left knee.
Employee was cleaning plant floor. When he stepped on a bolt causing him to slip and fall his left leg went under his body and twisted his left knee.
EE was climbing on loader to add engine oil to equipment when he said he pulled something in his hip.

EE complained of ""soreness"" on the left side of his ribs. EE was taken for an exam. It is estimated that the EE fell 5-6'. EE was told to take it easy for a while until he felt he was able to work.
based on med. Fx 5th metacarpal walking stumbling. there was no incident filed. ""As requested by MSHA, The information is being submitted by Brody Mining, LLC as a result of a Part 50 audit for PPOV"".
After employee drilled a hole for the roof bolt he was installing the glue in the hole when a piece of draw rock 18""x12""x3"" dislodged from the roof striking employee on right shoulder, scraping right side of lower back.
Employee removed wall from x-cut, went to clean up block where wall was. Rock fell from roof striking miner's mine hat and then slid on his back. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
Employee was walking to the bathhouse to get ready for work and stepped in a hole in the grass between building and parking lot. Employee fell and was helped to his feet and when he tried to walk his right knee buckled and he fell again.
Employee was rolling up the curtain for the miner when he stepped in a hole rolling his ankle. Employee went to doctor where he was ordered to remain off work for an uncertain period of time.
Placing cm timber jack on a scoop and the jack had grease on it which caused it to slip from his hands catching his left middle finger between the jack and the scoop. This required eight sutures to close and he was taken off work. First day missed was Monday, July 9, 2012.
Employee claims that he was descending the ladder down the side of the ore haulage truck when his foot slipped off the rung, causing him to fall and strike his left elbow and tail bone on the steps of the truck.

traveling in loader on haul road - tilt bucket dug into roadway and abruptly stopped the loader' s forward motion resulting in employee being thrown forward into cab cross member. Force caused cut on nose and broken nose.
Injured employee was using a utility knife to trim rubber from a screen panel when he accidentally cut his left Thumb. He was taken to the ER and received 2 sutures. He was released to return to work.
Employee heard the left rib pop and side stepped to the right and tripped over rock and fell landing on right wrist and injuring it. Employee started losing time on 8/3/2012 and is scheduled for surgery. This interns last day with JWR is on 8/10/2012.
He slipped on loose dirt lost balance and fell. EE broke his left arm.
Two EEs worked from a work deck suspended beneath the production shaft west skip, to replace a skip pocket guide. They asked for the deck to be lowered slightly quicker. The deck held up at tight spot & was struck by the skip, moving the deck approx 10' until the cables were taut, rapidly stopping the deck. The decks stopping jammed EEs into the deck causing their injuries.
DRIVER WAS HOOKING PUP TRAILER WHEN LOADER BACKED INTO EQUIPMENT CAUSING EQUIPMENT TO STRIKE DRIVER THROWING DRIVER APPROX. 20 FEET. DRIVER SUFFERED BRUISING AND SLIGHT CONCUSSION. PUP TRAILER SUSTAINED HEAVY DAMAGE TO FRAME.
IEE(injured) stopped water truck to ask another EE (mechanic) a question and at that time EE's were changing teeth on the Hitachi 330 track hoe. A jammed tooth required a hammer to loosen it. Mechanic hit the tooth with hammer when a piece of hammer broke off the edge and hit EE in the left bicep (which EE was standing 10' away)
Employee was exiting a locomotive and stepped on a piece of coal twisting his left ankle. Employee started losing time on 11-5-12
Miner was loading muck pile with Cat 992G loader. Discovered a ""toe"" under the muck pile and was using loader to break out ""toe"". Drove loader into an area of ""toe"" that refused to move causing a very sudden and abrupt stop. This lurched the miner in his seat causing an acute neck and upper back strain.
When placing 2nd dipper into truck, rock rolled off dipper, striking the canopy. Driver left shovel pit, and later reported feeling pain in her upper right back and neck. She was taken to her doctor and taken off work. Currently ongoing.
Miner installing belly pan underneath Cat D-9 dozer. Chain being used to hold pan in position slipped pinching miner' s hand between pan and dozer undercarriage. Left little finger was fractured.
The track crew was preparing to lay track on a continuous mining section. The injured employee was handling the track ties when he slipped and fell to the ground. He tried to brace himself with his right hand, causing a fracture to his right hand.

Employee was exiting a articulated Volvo haul truck and mis-stepped on the ladder. The employee suffered contusions to the L. elbow & shoulder. A potential fracture to the L. elbow is possible, pending further examinations.
Employee was changing a refuse screen in the prop plant. The screen jumped forward striking his wrist (right) and ankle.
Employee was getting onto the truck he was operating when the second rung on the ladder broke. this put all of the employees weight on his shoulders.
Employee was scooping for belt move when a loose rock fell and hit his right forearm requiring 8 stitches.
While setting 5 X 5 posts one came down and struck him in the wrist.
Conditions of the mine site had not changed since the last MSHA inspection. Conditions were normal. The severity of the operators injury consists of a severe laceration to the tip of his left index finger. Stitches were required.
Employee was operating an Euclid R35 haul truck. He lost control coming over a haul road and struck a berm. Operator stated he did not have any brakes. He also stated that his brakes did not feel right while he was being loaded. However, he did not notify anyone or park the truck until a mechanic could come look at the truck.
Carrying a prop-setter with co-worker, co-worker dropped his end and EE said that it pulled a muscle in his back. He did not want treatment but that he wanted to go home for the day. He called after the holiday and said that his back still hurt so we sent him to the doctor.
Employee was walking from the end of track toward the face. He stepped in a hole and pulled a hamstring in left leg.
As employee lifted up rock out of roadway, he twisted his trunk, hurting back
Individual operating 20 ton motor going from 8-5 to 7-5 on main north haulage. Trolly pole came off wire cawing the supply trip to drift back striking his motor. Individual landed on motor deck and hit left shoulder. Individual continued to work until 9-21-12 when he had surgery to the left shoulder.
EE was carrying a beam down the longwall face. Slipped and fell over the toe of a shield landing on his left wrist. Began losing time 7/18/12.
EE was putting bends in brackets using the chicago break machine. EE pushed the ""clamp"" button instead of the ""inch"" button smashing his left pinky. EE was taken to the ER. EE's tips of pinky bone chipped, pinky placed in splint and referred to Ortho. Taken off work.
Employee was welding on a loader when he twisted to get to the work spot to weld he felt pain in his neck.
The miner was emptying 100 lb. bags of material from a pallet elevated over a waste hopper. As he moved a bag of material to the front of the pallet, to empty the bag, the weight of the bags broke some of the pallet slats. This caused three bags of material to fall from the pallet and one of the bags struck the miner on his right leg and ankle, twisting his ankle.
EE was blowing off the dryer and felt the heat by his hand so reflexively pulled his arm back bumping his right elbow. He did not report it until the next day and declined to see a doctor. IT was still bothering him on 7/9 so we sent him to a doctor. He missed work on 7/9 and returned to work on 7/10.
The employee was loading mono-rail trucks onto a trailer. He had loaded 14 trucks onto the trailer and as he went to load the 15th mono rail truck, he felt a sharp pain in his lower back.
Lifted an oxygen bottle to put on his shoulder when he felt a pop and pain to the back of his left shoulder. Employee missed his scheduled shift of 8/10/12 for shoulder pain.
As the employee was standing on a ladder to hang a ventilation curtain, the ladder fell over resulting in the employee falling to the mine floor. He suffered a dislocated left ankle.
While changing a tire on a 785 rock truck finger were mashed between the lock ring and the bead seat of the wheel.
EMPLOYEE WAS PULLING CABLE THROUGH A RACEWAY FROM LEFT TO RIGHT. HE FELT A SUDDEN PAIN IN HIS LOWER LEFT LUMBAR REGION. DIAGNOSED AS A LUMBAR STRAIN.
3 maintenance EE's were working on replacing an O-Ring on a Dozer. One EE bumped the engine to force the fuel out of the cylinder, when the fuel entered the IE's right palm but did not exit out the other side of his hand. The pressure created 2 small cuts 2 inches long on different locations on the palm of the hand. Unknown where IE's hand was positioned when the injury occurred.
Walking toward vehicle slipped in mud - jammed right arm against vehicle - fell back and grabbed vehicle - pulled arm. Did not seek medical attention until 11/8/12 - off since 11/8/12.
Crew was installing additional 4"" line to tie into the existing 6"" air line, when they were unable to loosen the 2"" and the 3"" coupler from the ball reducer they started to remove the 6"" coupler. As the second bolt was loosened, the coupler blew off striking the employee in the left knee.
The employee stated that he had moved over to the center of the entry behind the miner to check his sights. A rib roll occurred inby of the last row of bolts and a piece traveled outby and struck the employee in the ankle.
Employee moving an electrical motor from #4 screen house to door. He felt a slight burning pain in left groin area.
While walking down the stairs, two steps from the bottom/or landing his left ankle twisted causing him to fall and while landing on the floor, a hammer that he had on his right hand hit his right leg on the thigh.
Lack of communication. Crusher was locked out, screening plant running. Victim apparently did not hear or heed warning from crusher operator, walked under crusher. Co-worker was removing rock threw off crusher struck EE as he emerged from under crusher. He was wearing hard hat.
Employee was using a sledge hammer to straighten out bent metal on lid of battery box resulting in lumbar sprain. Employee did not report injury until 7/30/12.
Employee was pushing pallet across concrete floor when pallet stopped abruptly, jamming his left wrist causing pain and a small knot on the back of his hand.
EE WAS WORKING ON THE FINISH MILL FEED CONVEYOR REMOVING CLINKER BUILD UP FROM UNDER THE BELT. HE WAS PRYING A PIECE OF CLINKER WITH A SHOVEL AND ANOTHER EMPLOYEE WAS ON THE OTHER SIDE DOING THE SAME THING WHEN HIS SHOVEL SLIPPED HITTING EE IN THE LEFT KNEE.
While removing the grill cover on a Cat D11 Dozer, the hinge broke causing the grill to fall onto the injured's hand breaking one finger and lacerating this hand requiring stitches.
EE was bolting in #5 heading. He just put up outside bolt swinging to inside when a piece of rock fell from the top and struck him in the foot.
EE was using a 3""x3""x1/4"" angle at 6"" long as a pry bar to lift an 8' H beam at 30' long. His back popped causing him to seek medical attention right away. Doctor subscribed 3 medications for spasms and wants to see him again in two days.
Employee states he was injured when he was carrying a pump, tripped and fell on his left knee. Employee became a lost time injury due to surgery on 8/28/12
Miner was loosening a bolt on a hydraulic rock drill. The wrench slipped off the bolt, causing the miners arm to hit the drill.
While drilling a hole in a guard, the drill bound-up twisting the miners right hand and wrist.
Employee was lifting screens in the quarry and strained his lower back causing a herniated disc.
Employee was troubleshooting a boarding ladder that was leaking off. The cover that was lifted to access the hydraulic pump area fell closed, smashing 2 fingers. The cover was 24""x24"" and weighed about 20lbs.
Employee was attempting to wash out rock box at #5 Belt Head. When Employee turned on the fire hose the pressure knocked him down causing injury to his left shoulder area. Employee was transported to Raleigh General and then due to the nature of injury transported to CAMC General Division and was diagnosed with a broken clavicle and admitted for observation.
Installing belt on crusher pulled back no witnesses notified on 7/26 was going to be checked out at hospital doctor said pulled muscles gave paid meds. sent medworks 7/31/12 sent back with RTW slip no treatment needed. Did not report at that time appeared treatment was diagnostic and preventive. Called in 8/16/12 in pain did not come to work.
Employee was installing idlers and strained left shoulder.
He was riding on a mantrip when his vehicle was struck by another mantrip. He felt pain in his back. *We dispute the allegations in Citation #8055886 and submit this report solely to terminate the citation.
Employee was hauling load of rocks when rock fell out of bucket on machine and caused him to run over a 24"" rock which caused the loader tire to blow out. In trying to recover, he lowered the bucket to ground which caused machine to go unstable and violently shake. Employee's body was jerked around. He exited machine and collapsed on ground.
Had just repaired the drill and was watching it drill to make sure repairs worked when a hose blew apart. Took off running to get away and blew out left knee.
Employee was shoveling clay from floor when he felt a pain in his lower back while twisting and bending.
Replacing fuel filter on D10T and the access door closed with enough force to crush his fingers on his left hand.
EE felt pain in abdomen while at work, but when reported, was not sure of activity that caused discomfort. Physician diagnosed as hernia, and EE underwent surgery on 8/16/12.
At apporx 07:45 operator EE while dismounting the CAT D9R dozer twisted his right knee. He was taken to the clinic and released to LLD until his follow up appt in a week.
The employee was throwing a track switch in the rail yard. He felt a pain in his ribs. He was diagnosed with a collapsed lung.
While employee was kneeling on wooden floor assembling a partition, an apparent sliver entered his right knee, which later that night caused swelling and pain.
EE was placing concrete forms on a pallet when one 2'x4' section slipped and hit his left leg. Doctor initially said was just bruised and to come back in one week. The next day the EE's leg was severely swollen and went to ER. Doctor said he had severed blood vessel in leg. Had to do surgery on 7/19/2012 to relieve pressure. EE will be out-of-work for 6-8 weeks.
Employee was replacing a belt on a motor on a locked out agitator. After replacement employee turned pulley causing his right thumb to be pinched between the belt and pulley. This caused a fracture of the bone at the tip of his right thumb.
EE was removing a tire and wheel from the tire machine, as he turned to his left to set the tire on the floor, he saw EE 2 was in his path, so he turned back to his right to set the tire back on the tire machine catching his two fingers between the wheel and the tire machine.
While digging in pile, the loader found some hard digging. When the loader bucket came loose, it caused the machine to bounce.
Normal mining conditions. Employee caught middle left finger between a roof bolt plate and the mine roof, crushing the tip of his finger.
Employee's left hand got pinched between the support frame & drill head on diamond drill machine. This resulted in a fracture to his hand.
Employee was walking up a stairwell, when he turned at the landing, he felt sharp pain in his right knee. He reported to the shift office immediately because he was having pain as he continued walking. EE placed on restricted duty on 07/16/12
While dismounting the last step of a track dozer the employee felt a pop in his knee when he put his weight on his right foot. He was evaluated at the ER the same day and released to work. He was seen by an orthopedic specialist on 7/24 and was diagnosed with a torn meniscus. He underwent arthroscopic surgery to repair the tear on 7/26.
While cutting a conveyor belt, employee suffered a laceration to his right forearm.
A general laborer fell into the surge pile while cleaning materials from the feeder opening and became trapped.
EE stated that while bolting, he extended the boom and it caught on the track and threw him upward. When he used his right arm to brace himself he felt pain in his right shoulder and elbow.Reportable lost time injury as of 8/15/12 due to surgery.
Employee was loading rock with Continuous Miner, when a rock was thrown from the CM and struck the operator in the face.
Was supplying glue to bolter-putting box of glue in tray hurt back while lifting.
Employee sought medical attention on 7/18 for a strained shoulder that occurred when he released a brake on a railcar. He was released to full duty. He had an MRI and an app't with an Orthopedic surgeon on 8/16 revealed a torn rotator cuff. He is currently on restricted duty while awaiting surgery. This is being reported within 10 days of being placed on work restrictions.
Employee was moving drill steel from the bed of a truck when his left, middle finger was pinched between the drill steel and the hand rail of the truck.
Furnace attendant was pulling a washdown hose that was coiled up on the north end of the dribble belts. He was pulling the hose down the walkway to hose out the 1B feeder. As he was pulling the hose out of the mud the hose became tangled. The employee then pulled and resulting in left shoulder pain
Employee was stepping across barge. It was raining and was slippery. He slipped and hit his knee on the barge. He has a contusion to his knees
A 22 ton rough terrain crane was traveling up an incline on a dirt access road when the crane stalled, lost power and steering, rolled back into the berm on the dirt road, turning over the crane and trapping the operator inside.
Employee was carrying a box and the box began to slip. As the employee grabbed the box, he felt a pull in his upper right arm. Restrictions began 08/02/2012.
An employee was in the process of adjusting the roll of shrink-wrap when one end of the bar holding the shrink-wrap slipped off the stand, this caused the roll weighing approximately 100 pounds to drop the opposite end of the bar raised crushed the employees right little finger between the bar and the stand.
He was changing an axle on a 5 ton motor. As he rolled the axle in place he felt a pain in his groin area. He was diagnosed with a bi-lateral hernia and began missing work his next scheduled shift (7/10/12).
Rock stuck between blade and liner of dozer; employee used sledge hammer to dislodge rock. Rock came out and hit his left leg, cutting it.
EE was walking down steps and he thought he was at the last step. But he was on the second to last and when he stepped his knee locked up and he couldn't bend it. He tore his Patella tendon in the left knee.
Employee was knocking out block around a Regulator, when he struck a glancing blow on the block and got his left middle finger caught between the hammer handle and the edge of the Regulator.
Employee experienced pain in his right shoulder while removing a cable from a barge July 7, 2012. On October 2, 2012 he was placed on restricted duty. This incident is being reported within 10 days of employee being placed on restrictions.
I send 2 employees to clear a area of logs. I told them not to lift anything. Send 2 one to hook chain one to unhook chain Cat man was up then one of the employees lifted a branch.
The employee operated the loader over the service road. The rough road may have caused a strain on his back and left hip. He reported a pain to his back & hip after operating the loader. This did not get reported until 8-15-2012. The employee went to the doctor on his own before reporting this injury/pain to his manager.
The employee stepped out of the cab of a Wagner 20X shield hauler and then reached into the cab and operated the fork down function. The machine articulated with the employee in the hinge point.
Mechanic alleges that while he was washing a truck in the mine wash bay, his foot slipped as he took a side step causing him to lose his balance. He states he did not fall down but felt pain in his lower back as he regained his balance. This incident is still under investigation.
Employee was digesting a sample using nitric acid, as employee was removing beaker it became sticky to glove which resulted in losing control of the beaker and dropping. The solution splashed up and resulted in 2nd degree burns to portions of face, neck, and arms.
Employee stepped out of truck and slipped on loose ground.
Miner operates a WDM Drill, noticed drill bit was dulling, went to Drill Pad area, to change out bit. Miner struck old bit w/2 lb hammer due to being stuck, metal hammer wasn't strong enough and a piece broke off, imbedding itself in upper G.I. area of miner, with downward angle. Piece had to be surgically removed by Specialist/Surgeon. Surgery successful on removal.
Employee was moving some large rocks into a pile on a bank. He set his hand down and one of the rocks rolled off the pile onto the rock his hand was set on, cutting the inside of his left pinky finger to the bone.
(See Attached) EE loading powder truck with 50 lb. bags of ANFO. EE stated he had aggravated his lower back and needed to go home and lay down. EE RTW 7/3 and worked full shift. We did not work on 7/4. EE came in 7/5 prepared his truck and started work. At 7:30 a.m. EE came to my office and stated he would make a doctor's appt. because his back was still painful.
Employee was walking down a flight of stairs on or about 7/2/2012 and felt a pain in her knee. Employee was seen by a doctor on 7/3/2012 and returned to full duty. After additional complaints the employee was returned to the doctor on 7/16/2012 for further evaluation and was removed from work.
EE was attempting to open the outer elevator door at the Primary Crusher when she felt pain and discomfort in her lower back, radiating down her leg.
Lubricator had just checked oil on vent fan bearings. He climbed down the vertical access ladder and was standing on an 11"" high concrete pad at the base of the ladder. He turned to leave, his foot rolled off the corner of the concrete, causing him to fall, landing on his elbow. He received a cut which required 3 stitches, was placed on restricted duty for 6 days
When tearing out refractory a spring on the rivet buster broke. Employee after being told not to use lower exit climbed down the ladder, knocked off the rivet buster from the deck hitting himself on the upper shoulder and back.
EE was hosing out a precip. hopper with the grating cover propped up. The cover fell down onto her left hand. She was evaluated by a physician at the end of her shift, diagnosed with a fracture and is working with restrictions.
Employee was driving a 769D haul truck and claims that he hit some ruts pulling a muscle in his abdomen.
Employee was working at the top shaft station. A crew of painters were doing elevated work out of a man basket adjacent to EE's position. The painters pulled some slack in a hose which caused it to dislodge a piece of angle iron which fell and hit a sloped roof, deflecting it and causing it to hit employee on the helmet. He incurred a neck strain and a laceration on his elbow.
Employee was removing a guard when it bumped his chest causing bruised ribs. Was seen by clinic on 8/2 - light duty until 8/6.
Breaker bar broke causing strain to right shoulder
Employee was assisting with moving a roller frame and his fingers were caught in a pinch point. He was diagnosed with two tuft fractures and released to restricted duty.
EE stated that he lifted a chock block. When he went to put it on the end dump, he stated that he felt a pain & pop in his lower back & pain in his abdomen. After investigations & doctor visits, it was determined that this was an on-the-job injury. He is working restricted duty.
EE was operating a scraper using the Push/pull technique and while doing so he ran over a rock and jarred his back. We took him into the doctor and he was put on restrictions of no lifting over 20 lbs and not sitting for longer than 20 min thus constituting in a MSHA reportable accident.
Employee was stepping over eight inch pipe and twisted his left ankle. Employee was examined by doctor and diagnosed was a sprained ankle. employee was returned to work with no climbing restriction and will follow-up with doctor on 8-8-12
Employee was shoveling sand away from belt area of screen and injured lower back. Not sure how.
Employee was removing what he thought was an oil fill cap to add oil to an air compressor. The air compressor was shut down before employee removed the cap. The cap was in actuality a cap to a check valve. When employee removed the cap the cap struck the employee on the nose bridge of his safety glasses. The safety glasses cut the bridge of his nose.
An employee was working on a belt press in the wash plant and fell from an elevated position, striking his head. The injury resulted in a mild concussion.
The employee was walking down the wash plant screen stairs when he slipped and went to catch himself and hurt his hand. Employee was sent to Fast Med, was released back to work on 8/1/2012 he experienced sharp pain and numbness and sent him back to Fast Med.
Miner was moving vacuum hose while cleaning spilled material from the ground when he noticed pain in his back.
While traveling to the section on the Rubber Tired Mac 8 Diesel powered mantrip the EE was thrown into the seat in front of him when the mantrip struck an embedded rail in the roadway causing it to stop suddenly. He sustained a hairline fracture to his 9th left rib. The EE ran his shuttle car on his next scheduled shift, so the company believed there was no restricted duty.
Per statements IEE who is an experienced haul truck op. was training another EE on a haul truck. EE was on 2nd day of training as he was backing up to a loader operator had the bucket raised so the truck could be loaded, as the truck got into position apparently the bucket was dropped quickly which jarred the truck and hurt his lower back. EE is on light duty.
After passing a jack hammer under a conveyor belt the injured felt pain in the area of his shoulder blade when he stood up. Diagnosed as thoracic muscle strain.
Scraper operator was driving through a ditch to get to a topsoil pile. We hit the ditch and felt pain in his lower back. EE was placed on restrictions on 8/1/2012 that affect normal work operations, thus constituting MSHA reportablity.
Employee was running from angry bull while removing it from thickener.
EE was working with another EE to jack up the cab of a 980 loader in order to access a hydraulic pump. The loader was being raised implementally with two jacks. During the lifting process the EE noticed a wire was caught and reached in to free it. The jack slipped and caused the cab to drop and pinch the EE's arm.
As the employee was dismounting a transportation carrier he fell to the ground causing a fracture to his right wrist.
The Employee surveying on the tailings pond 7/26/2012. A strain of the neck occurred the evening of 7/26/2012 the employee stated her neck and shoulders became tense and tight. when she awoke on Friday morning she said she was having pain and spasms in her neck. The employee reported this to supervisor on 7/31/2012 and went to the doctor on 7/31/2012.
An employee was climbing into a rubber tire dozer and felt a pop in his left shoulder .The employee was transported to Nye Regional Medical Center and was released with temporary restrictions.
Employee was removing hydraulic cap bolts (cap was pressurized with oil) and one came loose. The hydraulic pressure cut his work pants and skin on his right knee/shin. He went to MedExpress on 7/29/12 and was released to modified duty; no bending stooping or kneeling until follow up appt 8/4/12.
On July 25, 2012 just after midnight, a Precipitation employee working on overtime sprained his left ankle when it twisted under him when his right leg fell through an opening in the floor plate on top of number 6 Precipitator.
Employee reported that while attempting to remove a broken weir board from a sluice box, he slipped and fell down the bank of an empty dredge pond. Injuries were contusions to back and ankle and ankle sprain
Employee was reaching up to release a locking mechanism when he felt a strain to his shoulder. He was seen on a monthly basis until it was determined by the doctor in January to repair his shoulder. He was put on restricted duty after his surgery.
The employee was using a sledge hammer to clear dust from a hopper. He believes he strained himself swinging the sledge hammer.
Miner was repairing a PTO unit that has been removed from the plant when he was adjusting a part, the part slipped, struck his finger and he sustained a contusion to his right middle finger.
While riding on the personnel carrier on the way out of the mine, the man struck his head against the roof.
Moving a piece of 1/4 inch plate to access with toolcar and while trying to slide plate, braced arm, elbow against knee and while pushing, heard a pop in elbow.
Employee was rebuilding a water pump and applied a lot of pressure to remove the impeller. His left wrist was strained in the process and he was in pain. When he was seen by the physician, the injury showed possible signs of Carpel Tunnel Syndrome. He was issued a wrist splint to wear for two weeks and was placed with limited/restricted use of his left hand.
EE stepped off a step, approximately 1.5 feet, and felt a pain in his right hip. Treating doctor placed EE on restricted duty.
Employee was cleaning a over flow from #2 packer bin on the floor at the base of the bin. Employee claims lime dust may have entered his boot at a separation in the laces and tongue. The lime material caused chemical burn to the employees 3rd and 4th toes on the left foot.
The employee, while dismounting a lift, twisted his ankle at ground level. The employee was diagnosed with a sprained right ankle. The employee is on work restriction and has been transferred off the mine site.
While operating a ram car drove over a sulfur ball in the roadway and bounced into the canopy jamming his head and neck. Diagnosed as a neck strain and restricted to no driving of equipment until seen again by his physician.
Worker was carrying a 35lb stone, when he twisted to put it on the stack he felt his left hip ""pop"". Left hip strain. Restricted duty, No lost time.

Employee was opening the hood on his haul unit in order to check the oil level during his pre-operation inspection. He bent at the waist and threw the hood open. He felt a sharp pain in his back near his hip and felt pain when walking.
The operator of the 597 haul truck twisted her ankle on a wet step, causing a fracture.
Employee was exiting 980G loader. As he did this, his right foot slipped and went under the step, causing employee to fall to the ground while his left foot stayed on the loader step and hands slid down rail. As this happened... it hurt his left knee.
Employee, with the help of his driller, was lifting and pushing a core barrel into the drill chuck. When they were finished with the task, the employee said he felt a sharp pain shooting down his left side.
While lowering cathodes into the press a sheet got stuck. Employee raised the cathodes and grabbed the stuck sheet with his left hand. The sheet fell and caught employees hand cutting him between the 1st finger and thumb. Employee received stitches on his left 1st finger.
Employee slipped on wet stairs and slid down 11 stairs on his backside.
Employee was stepping up to bolter cab when he felt a pop and experienced some pain in his right knee. Over the next several weeks pain did not improve. MRI revealed a small meniscus cartilidge tear. This was surgically repaired on 09/19/2012.
Employee was attempting to remove a pin from a link for the drag conveyor with a hammer. A piece of metal came off and went into his left wrist.
Float Crew Employee attempted to cross over matrix line to inspect pump for leak. Employee stated he sat on pipe and threw both legs over the pipeline. While attempting to stand back up, his right foot slipped and turned over sideways.
Employee was installing a bearing on a conveyor head pulley shaft, he slid the bearing onto the shaft and was using a chisel and hammer to adjust the bearing. While hammering on chisel the employee felt something strike his left wrist, when he examined his wrist he could see a piece of metal embedded in his wrist.
EE WAS INVESTIGATING WHY DUMP TRUCK TAILGATE WAS NOT OPENING. EE TRIED TO REMOVE THE AIR CYLINDER BY HAND. THE CYLINDER ACTIVATED WITHOUT ENGAGEMENT PINNING HIS RIGHT HAND BETWEEN THE CYLINDER AND THE TRUCK BODY.
Employee was cutting a 2"" hose with a utility knife. The hose slipped. The knife slipped and he cut his left little finger. Four stitches.
Employee was rolling up an air hose and descending from a staircase. While on the last step, he stepped back forgetting he was on the stair and fell onto his backside and then on his lower back.
