Hoist power problem due to power company signal system to the hoist sub station. The Westinghouse 4160 switchgear dropped out when it recieved the signal, the unit worked properly. The hoist was out of service for more than 30 minutes.

Roof fall, 4th subpanel west (old #3 panel). Approx 110' long 18' wide 6-7' high.
Shipping approx. 120 diamond drill rods to surface from 5900 level. rods are 10' in length. somehow, at least one of the rods dislodged and hit the crown set on the 500 level station causing wreck in shaft. rods went down shaft and cage stopped at 4900 level station. impact caused damage to sets in the shift. incident is still under investigation. no injuries.
PAGE PORTAL SERVICE CAGE WAS SHUT DOWN FOR 4 HOURS TO DO REPAIR TO THE LOWER BULL WHEEL.
A roof fall occurred measuring 17'x 17'x 5' beginning at #4 panel off #2 entry at spad #3407. No equipment was involved, no ventilation was disturbed & no injuries, the fall will not be cleaned up.
A roof fall occured in the South West Vent Entries at crosscut #18 in the #6 and #7 entries. The fall measured 70' X 20' X 6'. This area was mined in August of 2000 and was supported with 6' tension rebar bolts and timbers. The fall was caused by an undetected slip.

A roof fall occurred on the 2L2L4W panel between cross-cut 14 and 15 in entry #5. The cause of the fall was weak laminated roof shale commonly referred to as stack rock.
A fall of supported roof was found by examiner on above date and time. the fall was located in an intersection at #4 break, #4 entry, on east mains #3 belt. the fall measured 20'x20'x approx 18' high. no injuries occurred, no equipment damaged, ventilation was not affected.

ROOF FALL WAS DISCOVERED ABOVE ANCHORAGE ON 11/27/04 AT 1030 PM. FALL MEASURED APPROXIMATELY 16' X 18' X 15'. AREA HAD BEEN MINED DURING THE END OF 1987 AND ORIGINALLY BOLTED WITH 5 FT RESIN ASSISTED BOLTS. NO INJURIES/NO BLOCKMENT VENTILATION
ON NOVEMBER 26, 2004 JUSTICE MINE HAD AN ELEVATOR MALFUNCTION. THE ELEVATOR WAS DOWN FOR APPROXIMATELY 101 MINUTES DUE TO ICE ON THE DOOR SWITCH.
THE FALL REVEALED A MUD SLIP IN AN AREA THAT WAS RE-BOLTED BECAUSE OF ROOF BREAKING UP. IT WAS RE-BOLTED WITH 4' AND 6' FULLY-GROUTED RESIN BOLTS.
A ROOF FALL OCCURRED ON 1 RT SET (9 BLT) 5 & 6 INTERSECTIONS #7. FALL APPROX 6'HIGH X 35'LONG X 16' - 17' WIDE. AREA BOLTED WITH 6' RESIN & 6' INSTALL II BOLTS. TOP WAS SHALE WITH LAMINATED SAND STONE.
Slope hoist was inoperative for approx 1 1/2 hours due to a faulty relay switch.
A roof fall occurred between #3 and #4 entry of the 2nd MNE at survey station 1 and 40. The fall measured 40' long by 18' wide by 10' thick. This area had been bolted with 9' double lock and 6' fully grouted resin bolts. All approaches have been timbered.
Converter for hoist kicked out and ropes slipped. Changed encoder, 3 brush holders and cleaned communicator. Test runs made. Down Time: 12:45 a.m to 4:00 a.m.

An unintentional roof fall was found (2) breaks inby spad 49794 in the neutral entry 2400' outby the 003 MMU. The fall measured 40' long, 30' wide, and 5.5' thick, 5' tension bolts were installed in the area. The fall did not impead travel or ventilation, no personal injuries occurred due to the fall. The fall was barricaded to prevent travel.
Cont miner was mining in #1 room on unit 5. He had mined in approx 15' on rt side of face. The operator and ramcar operator saw a orange glow at top of miner head. This lasted only a few seconds and then disappeared.
A roof fall occurred measuring 20'long x 11'wide x 3'high, beginning in #7 entry 20'inby spad 225 north mains. No equipment was involved, no ventilation was disturbed, no injuries and the fall will not be cleaned up.
The hoistman checking overtravel shutoffs when the overtravel (at mine level) not reset. Overtravel on lilly controller still operable. Electrician went underground, reset the magntic overtravel switch by running magnet over the switch, it worked fine from then on. There was no damage, physical or electrical and the hoist was put back in regular operation at 5:00pm.

A NON-INJURY BUMP OCCURED IN THE 4 WEST TAILGATE, MMM-010 BETWEEN #7 AND #8 CROSSCUT IN THE #3 EXIT. VENTILATION CONTROLS WERE DAMAGED AND OUT 1 HOUR, WAS REQUIRED TO RESTORE A STEPPING AND SECTION REGULATOR.
ROCK FALL OCCURRED ONE CROSSCUT OUTBY SURVEY STATION #138 IN THE #8 ENTRY, MEASURING 20 FEET DEEP AND BETWEEN 4' AND 6' HIGH.
A roof fall occured in the MMU west 36 xc, #1 entry. The fall measured 50 ft long, 18 ft wide and 10-12 feet thick.

A ROOF FALL OCCURRED AT LOCATION #3 ENTRY 1 BREAK INBY SPAD 3772 WITH AN INACTIVE FACE AREA. THE FALL WAS 65' X 19' X 8' TO 10' IN SIZE. SUPPORT WAS 4' RESIN BOLTS IN MIXED STRATA WITH NO RIDER SEAM. THERE WAS NO PERSONAL INJURY OR MATERIAL DAMAGE. THIS AREA WILL BE BREAKERED OFF AND NOT REHABILITATED.
After emptying silo found some warm coal stuck in silo. Emptied and washed down with water hoses. No damage to silo or any structure or belts.
An unintentional roof fall occurred at the Galatia North Portal, at the 11 west longwall tailgate, 4150'. the fall was approx approx 20' long, approx 18' wide and approx 15' thick. Fall did not affect ventilation or passage of workers. the fall will not be cleaned up.
ROCKFALL WAS FOUND DURING WEEKLY EXAMINATION OF THE INTAKE AIR COURSE ON THE DATE AND TIME STATED IN ITEM 6.
BASED ON THE INFORMATION RECEIVED FROM THOSE PRESENT IT APPEARS THAT THE IGNITION WAS CAUSED WHEN THE SPARKS FROM THE CUTTING MACHINE (SHEARER), IGNITED AN UNKNOWN METHANE BLEEDER WHILE CUTTING HARD ROCK.

SAME SHIFT. THE HOISTMAN CALLED MSHA AT 7:55 A.M. TO REPORT HOISTS DOWN. HOISTMAN CALLED MSHA BACK AT 9:15 TO SAY HOISTS WERE BACK IN SERVICE.

FIVE FOOT HIGH TECH ROOF BOLTS HAD BEEN INSTALLED IN THE AREA AND 6' AND 9' SUPPLEMENTARY SUPER BOLTS HAD BEEN INSTALLED ALSO.

AN IGNITION OCCURRED ON THE 6 NORHT MAINS SECTION, MMU 088, MINING CROSSCUT (00) TO (000) APPROXIMATELY 75 FEET IN CUT AT 122 BLOCK. EMPLOYEE STATED HE SAW AN ORANGE/YELLOWISH FLAME TRAVEL FROM RIGHT TO LEFT SIDE OF HEAD, 8 TO 10 SECONDS DURATION. USED WASHDOWN HOSE TO EXTINGUISH. NO INJURIES OR DAMAGE.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE #2 BELT APPROXIMATELY 40 INBY SURVEY STATION #1213. THE FALL WAS APPROXIMATELY 5 FT HIGH X 22 FT WIDE X 30 FT LONG.
Glowing ember in rehect compartment was caused by broken scraper. The result was an explosion that blew out 4 explosion doors in the circuit. The scraper was replaced and doors were replaced.

A bounce occured in the tailgate of the 4 west panel, mmu 010 between #8 and #9 x-cut.
Mine Hoist Developed electrical malfunction of controls. Inoperative for appro. 4 hours
The investigation of a ground failure beyond installed ground support at 41W 8400 determined the 3:30 AM blast started the failure. Dimensions of the failure were 18'h x 25'w x 50'l. West dipping structures at the face were undercut by the blast. The MSHA Helena field office was notified by telephone on 11/12/2004.
ROOF FALL APPROX. 8-9'HIGH IN A 3WAY INTERSECTION. THE AREA WAS BOLTED WITH 6' & 9'INSTALL II BOLTS ALSO WITH 4' & 6' RESIN BOLTS. APPROACHES ARE 15' & 16'WIDE. TOP WAS LAMENATED GREY SHALE.
Roof fall occurred on the #1 unit in entry #3 a neutral- the fall was 20 by 20 by 5 1/2. The unit had gone through a slip earlier in the week. 12 ft cable bolts had been installed in other entries.
Roof fall 3800' from working face with less than 300' of cover. Fall appeared to be layered slate with coal rider seam 8"" thick about 10' above coal seam. This area was bolted with 4' - 6' - 8' bolts (resins). This area had about 20 cribs 4' apart and Hientzmans between cribs several jacks with beams. This area developed 12/30/03. Low cover, rotten top, rider seam.
AN ALLEDGED IGNITION OCCURRED ON THE 6 NORTH MAINS SECTION, 088 MMU, 0 ENTRY APPROXIMATELY 115 FEET. EMPLOYEE STATED HE SAW AN ORANGE FLAME 1 TO 1 1/2 FEET FROM LEFT COB, ABOUT 12"" IN DIAMETER, LASTING 1 TO 2 SECONDS. SELF-EXTINGUISHING. NO INJURIES OR DAMAGE.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #8 RETURN AIRWAY ENTRY, 7000' OUTBY THE ACTIVE WORKING SECTION LOCATED AT SPAD #0458. THE FALL MEASURED 20' LONG X 20' WIDE X 3-5' THICK. THE AIRWAY OR TRAVEL IS NOT AFFECTED BY THIS FALL. THERE ARE 2 RETURN AIRWAYS IN THIS AREA.

MINE PROPS WERE SET FLOOR TO ROOF & AREA DANGERED OFF. THE FALL IS LOCATED DIRECTLY ACROSS SPAD #3047 (#6 ENTRY) IN THE #7 ENTRY. ESCAPEWAY WAS NOT AFFECTED.
A rock fall was found at 4 am-4:15 am by the fireboss. the fall was on #1 unit int he intake at 10 & 90 and went from #7 entry to #9 entry. the area had cribs built and 12' cable bolts in the intersection of #8 entry. the fall will be cleaned up thru intersection 7 & 8.

A roof fall occurred on the 2L4W panel on the 3B belt at cross-cut 18 in entry #4. The cause of the fall was weak laminated sandy shale, commonly referred to as stack rock. The fall covered the 3B belt for a distance of about 30 feet. The fall was cleaned up the area re-supported and timbered.

CABLE WAS REPLACED & GUARDWIRE INSTALLED TO KEEP CABLE AWAY FROM WALL BRACKETS. ELEVATOR WAS RETURNED TO SERVICE 11/11/04 AT 4:30 AM.
A BOUNCE OCCURRED IN 3 WEST TAILGATE X-CUT 9 INBY TO LW, 4 WEST PANEL MMU, 010, A DISTANCE OF 50' TO FACE. VENTILATION CONTROLS WERE DAMAGED AND OVER 1 HOUR WAS REQUIRED TO RESTOR VENTILATION TO THE LONGWALL FACE. MSHA WAS NOTIFIED AND INVESTIGATED.
A roof fall occurred on the 1L4W section at cross-cut 14 from entry 3 to 5. A slick sided slip fell on the miner while mining entry 4-5 blocking retreat of the miner. This was at the end of this crews shift. Before recovery work for the miner could begin the roof continued to fall and fell from #4 entry over to #3 entry.

THIS CONDITION WAS BETWEEN 28+75 TO 29+00. THE TAILGATE IMPASSIBLE PLAN WAS IMPLEMENTED AND THE AREA MINED THROUGH AND MADE SAFE. THIS DID NOT EFFECT VENTILATION (DISTRICT OFFICE NOTIFIED).
AN EXAMINATION OF THE 3W TAILGATE FOUND DETERIORATING ROOF AND RIBS IN TEH #2 AND #3 ENTRY INBY X-CUT 8. THE LONGWALL TRAVELWAY WAS UNSAFE AND THE SECTION WAS NOTIFIED AND IMPLEMENTED TAILGATE BLOCKAGE PLAN. MSHA WAS NOTIFIED AND INVESTIGATED.
Elevator outside door had cable pulled out. The elevator was shut down to correct problem. Re-installed cable end and adjusted doors. Elevator was put back into service at 10:15 a.m.
A roof fall occurred in the 2 North Panel in #9 entry starting at cross-cut #75 and extending through the intersection of cross-cut #77 measuring 115ft. x 19ft. x 5ft. This area was mined in September 2004 and supported with 5ft. fully grouted roof bolts.This fall was caused by an undetected slip.
A small fire occured at the Pinon pit on a dragline. This eqmnt was idled approx. 1 yr ago, we had welders cutting the top off so that we could fill the house with ash, sparks ignited a sm grease pan under the gear case. Fire was extinguish in 1 hr 50 min. Small fire utilized fire extinguisher but re-ignited & called for water truck to fully extinguish.

The fall was discovered by the foreman at approx. 2:47pm. Approx.size of the fall was 27'long x 18'wide x 11'high.

The belt stopped and employee found rock fall in the #4 belt line entry, 28 breaks inby the Big#1 headdrive. The rock fall was estimated at 15'x20'x40' by the time top quit working and area was cleaned up the fall measured approx 15'x20'x70'.
Hoistman was filling out reports while running hoist, thought he missed cut-off bell and overspeeds limits had set. Overspeed limits didn't set, causing North skip to hit headframe cutting rope. Down 6:30 pm (11-2-04) to 2:20 pm (11-3-04).
While pillaring in #1 entry block #55. 1415 joy continuous miner got stuck. while getting crib block and chain to pull miner out top fell in on miner and blocking two mrs's.
Motor lead wire burnt in to - repaired in approx. 1 hour. Hoist went down at 1:55 p.m. MSHA was on job site so they were notified. Hoist was back up and running at 3:00 p.m. MSHA was still there on the property and they were notified.

THE S/R ELEVATOR WAS FOUND TO HAVE A FAULTY DOOR LOCK SENSOR RELAY. THIS PROBLEM WAS CORRECTED BY THE SHINDLER REPAIRMAN. DURING A SUBSEQUENT INSPECTION IT WAS DETERMINED THAT ALIGNMENT OF GUIDE RAILS WAS NEEDED AND A NEW ROLLER BRACKET ASSEMBLY.
Fall occurred at approx 4:30 am in the #6 entry 2 x-cuts in by spad #2096. Area bolted with 4' resin bolts and 6 by 6 plates. Fall measured 40 ft long 10-20 ft wide and 4 ft thick. Fall was timbered off.
An operator initiated start-up procedures on the #5 calciner. He started the exhaust and tempering blowers. An ignition occurred. While the unit was down, bunker C had vaporized and collected in the upper calciner compartment. When tempering air was added, it provided additional oxygen to an already hot compartment resulting in the ignition. Estimated cost to repair, $30,000.
EXCAVATOR OPERATOR WAS LOADING A 785 ROCK TRUCK WHEN THE WALL FELL STRIKING THE OPERATORS COMPARTMENT OF THE MACHINE.
The mine owner received fatal injuries when he was caught between a 980C Cat Front End Loader & a tractor trailer. The coal truck was having trouble driving up the steep haul road. The end loader was moving into position to allow the victim to hook the steel pull rope to both pieces of equipment. The end loader struck the victim resulting in fatal injuries.
Lifting a portable generator into a pick up truck and felt a pull in the groin area on the right side. noticed on bulge when showering that evening.
Rag employee was holding got caught on drill bit used to clean shavings, amputated top of right middle finger.
Employee was loading bolter supplies, moved plate pallet, and cut band on the pallet and the band flew up and hit him in the left eye. (EE WAS NOT WEARING SAFETY GLASSES.)
LIFTING ON EQUIPMENT PARTS AND PULLED SOMETHING
Employee of customer Alamo Transit Trucking Company caught left ring finger on truck, pulling finger from hand.
Crane was lifting conveyor into place when lifting strap broke and conveyor support landed on my right foot.
Finger became entrapped between nut adjustment and ear backup, while breaking pipe apart.
Employee was installing a roof bolt which contained the metal tag and wire containing ASTM information. As he was rotating the bolt to seat it into the chuck, the wire caught the little finger right hand, severing the finger. The remainder of the finger was later amputated at a local hospital.
Injured was in process of installing new conveyor belt on an existing system & placed hand in a pinch point. The shaft going through a roll of belt fell out of the stand that it was supported on striking injured in rt. hand causing crushing injuries.
HE WAS HELPING A CO-WORKER PICK UP A ROCK. HE STOOD ON HIS TIP TOES AND TURNED WHILE PICKING UP THE ROCK. HE FELT A PAIN IN HIS GROIN AREA.
EE was told to service belt head, while greasing belt head the grease hose got caught in chain and pulled fingers in chain causing the amputation of the distal part of the 3rd digit to be lost at 1st joint and stitches repaired cuts on the 2nd and 4th digits of right hand.
WHILE KNOCKING OUT A STOPPING WALL, A CONCRETE BLOCK FELL & SMASHED HIS FINGER. 2ND FINGER RT. HAND.
While installing a roof bolt (spot bolt), a piece of rock (12"" x 12"" x 1 1/2"") fell, striking injured on head, cutting forehead.
Employee was changing circuit breaker without cutting power ""downstream"". The hot wires touched and arched creating a flash burn that burned the employee's hand.
Employee jumped off tracks of the Lime 2400 dragline onto a dirt pile and sprained his ankle.

INJURED & HIS SUPERVISOR WERE STANDING IN THE INTERSECTION IN THE LAST OPEN BREAK IN NO.3 ENTRY PREPARING TO MOVE THE MRS'S WHEN A PIECE OF ROCK (APPROX 34""X20""X 3""-7"" THICK) FELL FROM THE MINE ROOF FROM BETWEEN ROOF BOLTS STRIKING HIM ON HIS BACK AND LOWER RIGHT LEG.
Employee was loading B.O. belt rollers into the bucket of a scoop (LST-5S). He picked up a bottom roller & slipped on some wet rock dust which caused a sudden movement & strained his lower back. He stayed on the job until the end of his shift & then was taken to the E.R. at Castleview Hospital. He was diagnosed with a muscle sprain/strain and taken off of work for 3 days.
A 3' X 20"" X 1"" ROCK FELL AND TIP END STRUCK THE EMPLOYEE IN THE BACK.
Employee was loading a tank of oxygen into his service truck and felt a pain in his lower back. The employee continued to work until 12/25/04 at which time he obtained medical attention and started missing days worked, at this time the incident became reportable.
INJURED WAS ATTEMPTING TO MOVE A 5-HP PUMP OUT OF THE SCOOP ROADWAY. AS HE LIFTED AND TURNED, HE FELT SOMETHING PULL IN HIS LOWER BACK.
Employee was helping to remove monorail from 3-B beltline. as he was dropping a section of monorail off the belt the flange struck his left wrist.
EE, injured, and co-workers had completed placing four (4) dragline bucket teeth on the shoe of the dragline. EE was in process of scooting them back when one tooth shifted, catching the top portion of his little finger on his right hand. He sustained a mashing type injury along with a broken bone.
EE WAS MOVING A HALF FULL DRUM OF ANTI-FREEZE FROM THE SHOP TO A HAUL TRUCK OUTISDE OF THE SHOP TO DRAIN ANTI-FREEZE FROM TRUCK INTO DRUM. EE WAS USING A SET OF HAND TRUCKS TO MOVE DRUM. DRUM SHIFTED TO RIGHT SIDE OF HAND TRUCK AND STARTED TO TIP OVER. EE LUNGED TO CATCH DRUM AND STRAINED LOWER BACK.
INJ EE WAS WALKING FROM THE YARD OFFICE TO THE MILL BUILDING, WHEN HE TWISTED HIS KNEE DUE TO UNEVEN GROUND. DID NOT BECOME REPORTABLE UNTIL 1/31/05 WHEN HE STARTED TO MISS WORK DUE TO SURGERY.
Worker was climbing down from a rock screen, worker slipped and tried to catch himself to keep from falling and felt damage to right hip.
Employee was walking to backside of burn table and he tripped on hose and caught himself with both hands, folding hands back. EMPLOYEE STARTED LOSING TIME FROM WORK ON DECEMBER 30,2004.
Employee stated he was shoveling spillage around the feeder and was backing up while doing the work. he stepped on a belt roller that was int he walkway and twisted his right knee. employee did not miss any work until 12-11-04.
EE WAS CAUGHT BETWEEN THE MACHINE AND THE RIB. OPERATOR DID NOT SEE HIM THERE.
Injured was leaving feeder area when shuttle car cable caught foot flipping injured and he landed on right ankle.
EE WAS WALKING AROUND END OF SHUTTLE CAR WHEN EE SLIPPED AND FELL, TRIED TO CATCH SELF FROM FALL AND HURT RIGHT SHOULDER. ACCIDENT OCCURED IN #7 ENTRY AT POWER CAR WHERE SHUTTLE CAR WAS ANCHORED. 1ST RIGHT PANEL
EMPLOYEE WAS CLEANING WINDOWS ON FRONT END LOADER. AS HE STEPPED FROM ONE PLATFORM TO ANOTHER ON THE LOADER, HE STRAINED HIS BACK.
EE WAS MOVING A WHEEL FOR A 15 TON MOTOR, ATTACHED TO THE HOIST. THIS IS A MANUAL HOIST THAT WILL ONLY LIFT OBJECTS. IT HAS TO BE MOVED BY PULLING ON A STEEL CABLE THAT IS ATTACHED TO THE HOIST. EE WILL HAVE TO HAVE THE HERNIA REPAIRED BY SURGERY.
EMPLOYEE WAS CARRYING A GEAR FOR THE SHEARER WHEN HIS BACK POPPED.
WHILE GAINING ACCESS TO MACHINE, EE WAS ON STAIRS PULLING HIMSELF UP BY HANDRAILS. HIS FEET CAME OUT FROM UNDER HIM DUE TO OIL COVERED WITH DIRT ON TRACKPADS. BODY WEIGHT WAS HELD UP BY ARMS. HE FELT A PAIN FROM HIS RIGHT SHOULDER. THIS CASE WAS TREATED AS A FIRST AID CASE UNTIL 1/13/05. AT THAT TIME AN MRI SHOWED A TORN ROTATOR CUP. PHYSICIANS ARE RECOMMENDING SURGERY.
Employee was fueling his dozer and while he was on the catwalk getting out of the cab he slippped and landed against the door frame hitting his right shoulder. It was early in the morning. The catwalk was damp and it was dark out.
Employee was visually inspecting pugmill as part of daily routine and reached to lift a piece of wood with one hand and lift an inspection plate up with the other when he felt a sensation in his back between his shoulder blades.

Individual had knelt down to work on a pump power plug at the 6#1 belt load center. As he twisted his body to stand up, he felt pain in his lower back and was unable to straighten up. He walked a short distance - still unable to stand straight. Fellow workmen brought him out of the mine.

EE WAS GOING DOWN THE STEPS AT THE SCALE HOUSE. THERE WAS FROST ON THE STEPS AND EE SLIPPED. WHEN EE FELL, INJURED TAIL BONE, X-RAYS DID NOT SHOW A FRACTURE.
Employee got out of loader, tripped, grabbed the rail but couldn't hold on and fell to the ground. He hurt his right shoulder and back.

EMPLOYEE WAS BOLTING ON MINER 8A SECTION, HAD STARTED MINING IN #2 ENTRY MINING FROM 82 TO 83 BLOCK. HAD TESTED AREA WITH SLATE BAR 5 TO 10 MINUTES PREVIOUSLY. CONTINUED TO BOLT. A PIECE OF COAL FELL STRIKING EMPLOYEE ON LEFT SHOULDER AND NECK. COAL MEASURED 2' X 2' & UP TO 10"" THICK. HE CONTINUED TO WORK UNTI 12/1/04. RECEIVED DIAGNOSIS 12/6/04.
Employee was helping the miner change places. He was lifting the miner cable and placing it against the rib out of the haulway when he hurt his shoulder and arm. He didn't miss any work until December 21, 2004 when he went to see the doctor.
Employee was cleaning up material in the vicinity of the primary crusher most of the workday. At or near the end of the shift, the employee experienced lower back discomfort. Six (6) days later, he reported the incident and completed a report, but declined medical treatment. Two (2) more days passed before he elected to seek medical attention (Dec. 02).
EMPLOYEE STATED - HE WAS MOVING REFRACTORY WHEN A PIECE HIT HIS HAND. (1ST LOST TIME WAS 12/16/04).

Miner was working at Jaw Crusher feeding rocks. A rock became dislodged and fell on miner's hand, crushing middle right-hand finger.
The employee had just completed welding on the panline in E-10. As he was crossing the handrail to get into the walkway, he struck his head on the shield tip. His first day off work because of the incident was 12/13/04.

Ee was in the process of edging a rock. as he was pulling the strap to move the rock, the rock broke and fell. as the rock was falling, it grazed the ee's right wrist. he experienced some swelling and tenderness. x-rays revealed no fracture or broken bones. ee sustained a superficial scrape on his inner right wrist.
Employee was climbing down ladder on 992C endloader when his foot slipped and he hit his right elbow on the handrail. He went to the doctor the next day but x-rays were negative. Elbow kept swelling up and hurting. Was referred to specialist who performed surgery on 01/19/2005 to remove some scar tissue and a bone chip.
ON 11/24/04 EE WHILE LIFTING STEEL PULLED MUSCLE IN ABDOMEN & DIDN'T REPORT IT. HE THOUGHT IT WOULD HEAL. REINJURED IT ON 12/27/04. MORE PAIN BUT THOUGHT IT WOULD HEAL. REPORTED IT TO OFFICE ON 3/24/05. PAIN WAS SEVERE AND NEEDED MEDICAL ATTENTION.

While removing the rear axle shaft from the wheel of a Caterpillar 623F Scraper, it suddenly became free knocking the worker down and landing on his left ankle, severely bruising left foot/ankle,
WHILE GETTING ONTO GRADER, EE SLIPPED AND FELL FROM THE GRADER LADDER TO THE GROUND, BREAKING HIS FOOT
POWERSCREEN CLOGGED. HE TRIED TO DISLODGE ROCKS WITH SHOVEL. A ROCK FELL ON HIS HAND.
Employee was standing next to a skidloader. The skidloader operator began to move the skidloader and ran over the employee's right foot.
EE walked up to the miner in #3 entry, a roof rock fell out from between two rows of pins striking EE on the back, hip and leg. Ee was transported to hospital by stretcher and ambulance.
EMPLOYEE WAS CHECKING THE CHARGE ON A SET OF BATTERIES IN 3I. HE RAISED THE LID ON THE SCOOP BATTERIES & WHILE HE WAS CHECKING THE CHARGE THE LID FELL ON HIS RT. HAND.
Employee stated he was walking underground, started to crossover shuttle car cable, tripped over shuttle car cable, causing employee to fall onto his left arm, causing fracture to lower left arm.
EE WAS CARRYING A TOOL BAG INTO HEADER AREA AND LOST HIS FOOTING. THERE WERE NO OBSTRUCTIONS IN WALKWAY. EE STARTED LOSING TIME AGAIN ON 12/14/04 DUE TO INCREASED PAIN.
Roof bolter operators were positioning a 4 1/2' roll of welded wire mesh screen in preparation for the bolting cycle. The mesh was positioned parallel to and under the last row of permanent roof support, when a roof fall occurred. The roof rock sheared off at the last row of permanent roof support striking injured EE.
EE WAS THROWING MINER CABLE OFF MINER WHEN EE FELT PAIN IN LEFT SHOULDER.
Miner was splitting stone with hammer and chisel. Chisel slipped and he hit his left hand.
Employee alleges he was driving a truck toward a sand pile and he hit a pot hole in the road causing his truck to bounce and injuring his neck, shoulder and back.
EMPLOYEE WAS WALKING THROUGH THE PREP PLANT WHEN HE STEPPED INTO A FLOOR SUMP AND BRUISED HIS SHIN AND KNEE.
EE WAS TRYING TO MOVE A DIAMOND WIRE SAW CONTROL BOX THAT WAS FROZEN IN THE GROUND. WHEN EE PULLED ON THE BOX, FELT PAIN IN BACK
EE PLACED THE S & S SCOOP LOADED WITH ROOF BOLTS CLOSE TO HIS ROOF BOLTER AND WAS STANDING BETWEEN THE SCOOP AND BOLTER.
Employee was removing a long bolt from a propel cam on the dragline. The bolt came out and the employee caught it with his left hand, pulling his shoulder. As he started down the stairs, he fell and ended up on the dragline shoe. He received a broken humerous, dislocated shoulder and a laceration to his right leg requiring sutures.
EE WAS ATTEMPTING TO BREAK BOLTS LOOSE ON A MOTOR WHEN HE STRAINED HIS RIGHT SHOULDER. REPORTABLE LOST TIME INJURY AS OF 4/20/05
EE WAS OPERATING A SUPPLY JEEP, HAULING SUPPLIES TO #1 UNIT. HE WAS TRAVELING THROUGH THE BACK-UP CURTAIN ON THE SUPPLY ROAD WITH THE OPERATOR'S DECK FIRST WHEN HE RAN INTO A DAPCO 5-MAN DIESEL RIDE PARKED IMMEDIATELY ON THE OTHER SIDE OF THE CURTAIN. THE FRONT OF THE RIDE CAME UP INTO THE DECK, CUTTING EE'S RIGHT ARM.
EE STATED THAT WHILE LIFTING A MINER CABLE HE FELT PAIN IN HIS NECK AND RIGHT SHOULDER.
While operating his ram car on the section, the injured ee's ram car bumped the mine roof and a piece of rock fell on him. The rock was approx 8"" thick and 4' long. Employee received contusions and bruises.
Was stepping off WA700 Loader. Employee was coming down the ladder, he stepped on a rock & twisted his left ankle.

Injured was attempting to break loose a plugged lime chute using a bar and hammer. When lime broke loose, it came through the 5"" port hole and hit injured in the face. It knocked the dust mask loose causing injured to breathe in a large amount of lime.

Employee was working on a conveyor structure when he fell through a gap in the catwalk. He fell 36' to the ground below.
THE EE STATED HE WAS ATTEMPTING TO FIND THE LOCATION OF AN EMULSION LEAK ON SHIELD #48. HE WAS RUNNING THE PAN PUSH FUNCTION WHILE RUNNING HIS HAND DOWN A HOSE, WHEN THE PRESSURE WAS ACTIVATED; IT INJECTED EMULSION INTO THE MIDDLE FINGER OF HIS LEFT HAND.

Injured had scaled roof down and installed first row. They moved bolter to install the 2nd row of bolts. As he positioned his body to insert the resin and bolt, he placed his rt. hand on the bolter boom and a rock weighing approx. 20 lbs. fell striking his wrist. NOTE: DID NOT MISS WORK UNTIL 01/18/05.
Injured was changing seats in shuttle car and caught his head between top and buggy, straining his neck. Injured went to hospital, was told he was ok. Company was told a week later his neck was hurting. Last work day was 12-2-04.

EE STATED THAT HE WAS PUTTING SUPPLIES ON THE ROOF BOLTER WHEN A PIECE OF DRAW ROCK FELL AND STRUCK HIM BEHIND THE RIGHT EAR. THE INJURY CAUSED A LACERATION BEHIND THE EAR REQUIRING 13 STITCHES. EE MISSED WORK ON 11-19 AND 11-20 THEN RETURNED TO WORK ON 11-22-04.

The employee picked up a pallet jack and while loading it into a pickup truck the handle assembly shifted downward and pinched the employee's left thumb between the handle and the jack assembly.

EE was troubleshooting a high voltage circuit when he came in contact with energized parts in an electrical junction box. He had chest pains and burns on both hands.
EMPLOYEE SLIPPED ON STEP AND TWISTED LEFT FOOT. EMPLOYEE WAS TREATED AT THE HOSPITAL AND WAS DIAGNOSED AS HAVING HIS LEFT ANKLE BROKEN IN TWO PLACES.
EMPLOYEE WAS PALLETIZING STONE. HE PICKED UP A PIECE OF STONE AT HIS RIGHT SIDE, LIFTING AND TWISTING AT THE SAME TIME TO POSITION THE STONE ONTO A PALLET DIRECTLY TO THE FRONT OF HIS POSITION WHEN HE SUSTAINED A SPRAIN TO HIS LOWER BACK.
Was filling a can of water out of fire tap to plaster stopping when a piece of rock 124"" by 16"" by 4"" slid off screen and struck him on right shoulder and neck and knocked down waterline.
EE stated he was in the process of examining the primary sampler for the clean coal loadout, while moving the metal that was next to the sampler cutter guard, the metal fell pinning him against the floor, causing fractures to both legs and his left hip.
Employee had his hand between two sections of hauler. He steered hauler, mashing his middle and ring finger on right hand.
Employee was helping to build seals in the Hatfield 1st left and had skin irritation from the B-bond sealant on his chest, stomach, legs and arms.
EE OBSERVED A FORKLIFT OPERATOR MOVING A BAG OF GILSONITE. HE DESIRED TO CROSS THE WORKING PATH OF THE FORKLIFT. EE WAITED UNTIL THE OPERATOR STARTED TO SET THE LOAD DOWN, THEN WALKED BEHIND THE FORKLIFT. THE FORKLIFT BACKED INTO HIM.
EMPLOYEE WAS MOVING A BUNDLE OF RESIN FROM THE BACK OF THE ROOF BOLTER TO THE FRONT, WHEN HE PICKED UP THE BUNDLE HE TURNED TO MOVE TO THE FRONT OF THE MACHINE AND HE FELT A PAIN IN HIS LOWER BACK.
Injured employee was on third step of six foot step ladder cutting the top pin holding a strut. Bottom pin was removed. When top pin was cut, bottom of strut swung out and hit ladder knocking employee off onto the ground. Strut was secured by a choker attached to a boom on service truck.
WHILE LIFTING MINER CABLE WITH COWORKER, EE STRAINED HIS RIGHT SHOULDER. DIAGNOSED 11/22/04 WITH TORN SHOULDER MUSCLE.

EE CAUGHT HIS FINGER BETWEEN TWO ROCKS WHILE MARKING OFF A SHOT - TRYING TO MOVE ROCK OUT OF WAY.
Employee was welding and received flash burns to both eyes. He was wearing protective equipment. A reflection from the water caused the flash burns.
Employee was pushing a radial stacking conveyor by hand and pulled his rib muscles.
Employee was helping reblock jaw crusher, and blocks gave way. Machine dropped 6"" and the cross member hit employee between shoulder blades.
EMPLOYEE WAS MIXING. THE GRINDER BECAME PLUGGED. HE CAUGHT HIS THUMB IN THE GRINDER. HIS THUMB WAS CUT ACROSS THE TOP AND DOWN ONE SIDE AND HE TORE HIS THUMBNAIL OFF. HE SAW A DOCTOR IN THE E.R. HE WILL HAVE A RE-CHECK 11/19/04 AND IS SCHEDULED FOR AN X-RAY.
Employee was pulling used beltline with an it-28 loader, backed up onto the berm, and turned the unit over. he was taken to the e.r. for treatment. x rays were negative, however he had a bruise on this right hip and the dr told him to stay off of his feet for a few days.
Bed of Dust Haul truck hit conveyor belt and turned haul truck on it's side.
Chute from 8'x20' screen to surge rock transfer conveyor plugged up w/material & had to be cleared. Once plug up was cleared EE & co-worker were walking under screen tower to haul trucks. A large wood block was on the concrete. EE saw the block & tried to step over it. He tripped & began to fall. He reached out & grabbed the side of the conveyor to keep from falling.
EMPLOYEE STATED THAT HE JARRED HIS BACK WHILE OPERATING A SCOOP. FELT PAIN IN LOWER BACK. DID NOT START MISSING WORK UNTIL 11/23/04.
EMPLOYEE PUSHED UPWARD ON A CONVEYOR GUARD TO FREE ROCK THAT WAS TRAPPED WITHIN. HE FELT BACK PAIN LATER IN THE DAY AND HAD DIFFICULTY CLIMBING INTO EQUIPMENT.
EE WAS GETTING ROCK OUT OF CONVEYOR CHAIN ON THE MINER WHEN EE FELT PAIN IN BACK AND CHEST.
EE crawled out from under drill, felt slight pain just below the knee, noticed a small abrasion. Finished his shift, was scheduled off on 11/17/04. Returned on 11/18/04, worked a full shift but the leg was swollen. Saw company physician on 11/19/04, diagnosis: cellulitis infection. Issued antibiotic and ordered to stay off of it. He returned on 11/23/04.
Employee was climbing onto a piece of equipment in order for him to operate said equipment. Injured shoulder in process.

Employee was bolting top when a loose piece of coal struck him in the eye.
EE WAS DRIVING TRACTOR TRAILER. THE ROADWAY TO THE REFUSE SITE HAD BEEN WATERED TO SUPPRESS DUST. THE TEMPERATURE HAD DROPPED AND THE ROADWAY FROZE PARTIALLY. EE WAS BLINDED BY BRIGHT SUNLIGHT AND HIS SPEED WAS EXCEEDING THE 10 M.P.H. LIMIT. THE TRUCK FAILED TO NEGOTIATE A TURN AND DROVE OFF THE ROAD, STRIKING A CEMENT WALL.
THE OPERATOR OF HAUL BACK TRUCK MISSED THE DUMPING AREA. HE BACKED OVER BERM WHICH RESULTED IN THE TRUCK ROLLING OVER ONE COMPLETE TIME LANDING ON ITS WHEELS. THE OPERATOR RECEIVED STITCHES ON HIS LEFT FOREARM & FRACTURED RIBS ON BOTH SIDES.
EE was repairing a flat tire on the rear of a low-boy trailer unit. The EE removed the tire and after making repairs was inflating with enough air to seat the lock ring, the lock ring blew off striking ee in the chin leaving a 4-6 inch cut. Received several stitches.
EE WAS BOLTING TOP, DRILL POT WAS STOPPED UP. EE BENT DOWN TO UNSTOP DRILL POT WITH A PIECE OF WIRE WHEN A PIECE OF ROCK STRUCK HIM ON THE SIDE OF THE HEAD, CUTTING HIS HEAD.
EMPLOYEE REPORTED: HELPING SECTION REPAIRMAN MAKE REPAIRS TO RAM CAR. WHEN ATTEMPTING TO REINSTALL THE PANEL COVER LID, HE TURNED WHILE LIFTING THE PANEL LID, EXPERIENCED PAIN TO HIS LOW BACK. CONTINUED TO WORK ALL SCHEDULED SHIFTS UNTIL 12/22/04. STARTED MISSING WORK ON THAT DATE, BECAME A REPORTABLE INJURY ON 12/22/04.
Injured was helping on miner, when a piece of rock was throws out of the boom, striking him in the hose.
EE WAS OPERATING A GETMAN TRACTOR. AS EE WAS TRAVELING ALONG, CAME UPON 10 TO 15 CONCRETE BLOCKS IN THE ROADWAY. EE THREW THE BLOCKS OUT OF THE ROADWAY AND STATED THAT HIS BACK WAS HURTING.
Ee was refilling glue box on the tray at the drill boom station. immediatelly after filling glue he placed his hand on the tray when a piece of draw rock measuring 44"" x 16"" x 0-3"" thick fell from the roof striking his finger. left hand causing lacerations.
EE WAS LOADING BELT STRUCTURE INTO A SCOOP BUCKET, EE LEFT LEG BECAME TANGLED IN THE STRUCTURE. EE LOST BALANCE, FALLING FORWARD STRAINING LEFT KNEE.
PUTTING CABLE ONTO MIDDLE HOOD OF MINER, STUCK FORWARD, TRIED TO GRAB & JERKED LOOSE. HOOK SWUNG AROUND & SMASHED FINGER.
Employee was descending a railroad slurry car ladder when his foot slipped and he fell to the ground. He sustained a sprained left ankle.
EE STATED EE AND COWORKER DISCONNECTED THE 4"" WATER LINE FROM DRISCO LINE, IN PROCESS THE WATER STARTED BLOWING OUT AND CAUSED EE TO FALL AND HURT SHOULDER.
EMPLOYEE WAS WORKING IN AN AREA OF THE PLANT THAT HAS CAUSTIC IN IT. THE EE HAD A HOLE IN HIS BOOT AND HE RECEIVED A BURN ON HIS BIG TOE.
INJURED WAS SPLICING BELT. WHEN PULLING ON BELT HE FELT PAIN IN THE RIGHT SIDE OF HIS STOMACH CAUSING A STRAIN.

EE WAS OPENING A BAG OF ROCK DUST LOCATED ON THE BACK OF THE BOLTING MACHINE WHEN THE OPERATOR STARTED UP THE BOLTER AND TRAMMED FORWARD, RUNNING OVER EE'S FOOT.
At the time of the incident, EE was installing a steering control valve on a 793 off-highway truck. While lifting the valve into place he strained the muscles in his lower back.
Employee has a history of back problems. During the week of 11/8/04 he worked many hours welding in tight quarters. By 11/13/04 the employee's back was hurting.
EMPLOYEE FELL OF OF A BICYCLE.
EE WAS HELPING ANOTHER MECHANIC REMOVE PULLEY FROM ALTERNATOR AND THE INPACT SOCKET BEING USED BROKE AND THE IMPACT WRENCH STRUCK THE EMPLOYEE ON THE BACK OF RIGHT HAND, RESULT WAS A LACERATION AND CONTUSION.
Employee picked up a 50 pound pump and threw his back out.
Employee, while assisting the ground crew get a sump pump out of a muddy area, strained his groin area. The employee was diagnosed on 11/29 as having a hernia and was scheduled to have surgery on 12/13/04. Employee continued to work with no restrictions to his job up until his surgery.
Employee was removing vent tubes when he twisted his ankle.
Employee was attempting to move the sliding line curtain up when he felt pain in his right shoulder. He worked up to 12/26/04 before he started losing time on his injury.
DISCONNECTING #3 PUMP MOTOR, EMPLOYEE CLIMBED OVER PLASTIC PIPES, WHICH ARE IN USE, TO GAIN ACCESS TO THE MOTOR. TWISTED LEFT KNEE AS HE STEPPED DOWN.
EE STRAINED HIS NECK WHILE SHOVELING. EE KEPT ON WORKING WITH NO RESTRICTIONS UNTIL 11/18/04. EE WENT TO DOCTOR AND WAS OFF THURSDAY AND FRIDAY (11/18 & 11/19). EE CAME BACK ON MONDAY (11/22) WITH NO RESTRICTIONS. I (SAFETY OFFICER) WAS ON VACATION WHEN EE WENT OUT. (EE IS A HUNTER.)
EE WAS CLIMBING INTO THE LOADER. HIS HAND SLIPPED OFF THE HANDRAIL, AND HE FELL BACK. THE FOLLOWING MONDAY, THE EE HAD SO MUCH BACK PAIN THAT HE WAS UNABLE TO WORK AND NEEDED MEDICAL ATTENTION.
EMPLOYEE WAS 1 BRK INBY THE FEEDER. HE GOT OUT OF HIS CAR TO HELP HANG A MINER CABLE. HE SLIPPED ON A ROCK AND TWISTED HIS RIGHT KNEE. HE WENT TO THE ER, WAS EXAMINED, AND GIVEN A RTW RELEASE. EE CONTINUED TO WORK, PAIN IN HIS KNEE INCREASED AND HE SAW THE DR IN LEXINGTON. HE HAD SURGERY ON 2-14-05 FOR A MENISCUS TEAR, AND RECOVERY TIME IS EXPECTED TO BE ABOUT 2 MONTHS.
Changing panels on stationary screens and slipped causing pain in groin. Injured continued to work until 1/14/2005 when he began losing time. HERNIA SURGICALLY REPAIRED.
TRUCK DRIVER WAS UNLOADING MATERIAL TO ADJUST HAUL WEIGHT AFTER DUMPING SOME MATERIAL OFF. DRIVER FOUND A ROCK STUCK BETWEEN THE TAILGATE & BED, SHE REACHED IN & PULLED ROCK OUT CAUSING THE TAILGATE TO CLOSE ONTO HER RIGHT HAND.
The ee was loaidng a bundle of glue sticks onto the bolter boom tray. As he tossed the bundle of glue onto the tray he also twisted at the waist and felt a pop.
Employee was washing material out of the bed of a truck and was struck by a 5"" piece of stone that was stuck in the corner of the bed with fine material. The stone rolled down the bed and bounced off a piece of rubber toward the employee striking him in the shin.
EE WAS LIFTING A FLAT PIECE OF STONE UP ONTO ITS EDGE TO BE SPLIT WHEN HE SUSTAINED A STRAIN TO HIS LOWER BACK. EE WAS SENT TO THE DOCTOR ON 11/11/04. DOCTOR RECOMMENDED NO WORK FOR EE UNTIL AFTER HIS NEXT EVALUATION ON 11/19/04.
While tramming a unihauler, the canopy caught a 1/2 header hanging down which broke the canopy loose and it fell off striking employee in the head and neck. Employee did not miss any work until 11/29/2004.
EE was operating a scoop when one of the wheels dropped into an undetected hole, causing ee to bounce upward and hit his head on the canopy. He experienced pain in his neck and upper back but continued to work thru Jan 17, 05.
EE stated he was pulling bolter cable slack up to power center and strained his back. last day worked was 11-2-04.
EMPLOYEE'S CO-WORKER BROKE A DRILL STEEL OFF IN THE OPPOSITE SIDE CHUCK. EMPLOYEE ATTEMPTED TO HELP GET IT OUT AND DECIDED TO TIE A NYLON ROPE TO THE MAST AND DRILL STEEL AND ATTEMPTED TO LOWER THE CHUCK. EMPLOYEE REACHED TO SECURE THE ROPE WHEN HIS THUMB BECAME TRAPPED BETWEEN THE ROPE AND THE DRILL STEEL.

After installing dump cylinders on the lower doors of a Supra Haul Truck, the employee was removing a support that was in place to keep the lower door from falling. When the support was removed, the door fell and struck the employee on the right leg.

Employee was installing a roof bolt that was bent a little in the head end. He slipped, causing him to hit the rotation level before he was ready. He had a hold of the pin and it spun around out of his hand and struck him on the thumb/wrist. He didn't miss work until 11-16-04.

AFTER LOADING A ROLL OF BELTING ONTO A LOWBOY, HE WAS WALKING ALONG THE TRACK ENTRY. THE LOWBOY WAS BEING PULLED BY A BUS. AS IT PASSED THE EMPLOYEE, IT STRUCK HIM ON THE LEFT ANKLE.
Employee was on his hands and knees collaring a hole. A rock fell from the face and struck the back of his left hand. He received a compound fracture to a metacarpal bone.
Employee was installing a 10 ft 2 piece roof bolt in the roof. He was holding the coupler with his right hand and using the rotation to tighten the two bolts together. His glove got caught in the coupler causing his right arm to be pulled toward the bolter. He receieved a severe right shoulder dislocation.
WAS HOLDING ONE END OF METAL POST AND BRACE WHILE IT WAS BEING CUT. POST FELL WHEN IT WAS CUT FREE, HITTING EE HAND.

Employee stated that he was working on 6 North Mains section. Was removing a broken fitting, while loosening vise handle he struck right hand between handle and top of tool car where vise was mounted, completed shift, began missing work on 11-9-04.
EE was uncoupling quick start coupling from 777B rock truck when he stepped on an uneven area of frozen ground, he lost his balance and twisted his left knee that had previously had problems.
EE WAS REPLACING A GUARD AT A PUG MILL WHEN EE STEPPED BACKWARDS AND STUMBLED CAUSING EE TO FALL. AS EE WAS FALLING EE REACHED OUT TO BRACE EE AND REINJURED A PREEXISTING WRIST INJURY.
Ee was driving a metal post with a hand post driver, he raised the driver too high, missed the post on the downward stroke and struck his left thumb between the post and the driver - fracturing his left thumb. (Leather gloves were being worn.)
Glove got caught in chain and hurt finger. did not break it or dislocate it just hurt. doctor put splint on it and sent him home.
Stepping down from ladder on loader when employee twisted knee. His foot evidently landed on a small stone or unlevel ground. Wasn't aware of severity of injury until later in day and it became much worse later in week. Eventually had to have surgery to repair torn ligament. Area was fairly smooth and covered with road stone and step was within 2' of ground.
RE-AGGRIVATION OF INJURY THAT HAPPENED ON 8/20/04 (COLLISION OF TWO MANTRIPS TRAVELING TO SECTION). ON 11/8/04 THE SUPERVISOR WAS SHOVELING BELT AND FELT PAIN IN HIS LOWER BACK. EXAM SHOWED A MUSCLE PULL TO SAME LOWER BACK AREA. THE SUPERVISOR WAS UNABLE TO RETURN TO WORK HIS NEXT SCHEDULED SHIFT.
EE WAS GOING INSIDE IN MANTRIP AND BRAKES FAILED. EE JUMPED OUT OF RIDE, JAMMED LT KNEE AND LT HIP WHEN EE HIT BOTTOM. HAD SORENESS BELOW RT KNEE WHERE IT HIT MANTRIP. EE STATED HAVING MUSCLE SPASMS IN HIP AND LOWER BACK.
5 NORTH FACE - AT FEEDER. EE WAS OPERATING SHUTTLE CAR, GOING TO FEEDER. HE REACHED OUT TO PUSH OTHER SHUTTLE CAR'S ANCHOR AWAY FROM HIS CANOPY. HE GOT HIS RIGHT HAND CAUGHT BETWEEN HIS CANOPY AND ANCHOR.
EMPLOYEE STATES HE SUFFERED EYE IRRITATION AFTER COMPLETING HIS NIGHT SHIFT ON 11/8/04. EYE PROGRESSIVELY GOT WORSE AFTER 3 DAYS & HE REPORTED INJURY TO SUPERVISOR ON 11-11-04. PHYSICIAN REMOVED FOREIGN OBJECT FROM RIGHT EYE.
Park brake leaked off manbus causing it to run away striking rib when it derailed throwing ee around inside bus.
Ee was installing roof bolts from the offside of the bolter. After drilling a hole he was installing a bolt. He said that he put the bolt up in the hole. He had his right hand on the bolt, he was reaching for control levers & he said that he slipped & engaged the rotation lever pulling him against the bolter. He was complaining that his right arm was hurting.
Employee and other co-workers were in the process of installing rail for the shearer stall. Cause of this injury was safety awareness and communication. Compliance with rules/regs not a factor, protective equipment not a factor, miner knowledge could be a factor, mining equip/systems not a factor.
SALARIED EMPLOYEE WAS SUPERVISING EMPLOYEES WHO WERE CHANGING LINERS AND FEED PIPE IN RAW MILL WHEN HE TWISTED HIS KNEE ON THE UNEVEN SURFACE. (DID NOT BECOME REPORTABLE UNTIL HE HAD SURGERY ON 1-18-05 AND STARTED TO MISS WORK.
EE got out of his truck to clean his tailgate and did not set his parking brake. The truck began to roll, ee tried to get back in the truck, lost his balance and fell beside the truck which ended up on his lower legs. He had severe bruising and contusions to this area esp right leg.

EE WAS SPREADING STRAW ON A BERM. COWORKER ROLLED A BALE OF STRAW TOWARDS EE. EE REACHED OUT TO CATCH AND STOP THE BALE AND HPEREXTENDED HIS LEFT ARM, FELT AND HEARD SOMETHING SNAP OR TEAR IN LEFT ARM ABOVE THE ELBOW.
Employee was lifting a heavy object in the normal course of performing duties.
EMPLOYEE WAS AT THE END OF THE STAGELOADER, ADJUSTING THE DOOR, WHEN THE STAGELOADER THREW OUT A ROCK THAT STRUCK HIM ON THE ELBOW.
EE was using a utility knife to cut fiberglass pipe insulation. The wind carried the insulation into ee's face. Symptoms were not immediate. A few days later, ee's eyes became swollen and he experienced respiratory discomfort.
EE notified us that he was going to the doctor for an incident that occurred on 11-7-04, ee stated he felt pain in his lower back while working on miner ripper head. EE has had ongoing back pain. Did not start missing work until 11-29-04.

SPLICING 13 BELT, EE WAS CUTTING BELT WITH A UTILITY KNIFE. KNIFE CUT THRU THE BELT, WITH THE KNIFE BLADE CUTTING HIS LEFT ARM, RESULTING IN A LACERATION. THIS IS A LTA.
Employee was applying Combowear 3300 Resin and experienced an allergic sensitization/reaction.
Employee was struck on the hard hat by a bar. Employee received a laceration to his scalp requiring sutures.
EMPLOYEE WAS STRUCK BY A 3"" WATER LINE WHEN A COUPLING FAILED UNDER PRESSURE. THE COUPLING HAD BEEN LEAKING. WATER WAS TURNED OFF, COUPLING WAS TAKEN APART, CHECKED AND TIGHTENED. THE WATER WAS TURNED ON SLOWLY. COUPLING HELD FOR THREE MINUTES AND THEN BLEW APART.
EE was lifting a steel plate in the plant and felt a pull in his groin. At the time he thought it was just a pulled muscle and did not need any medical attention. On Dec 1, 04 ee went to the Dr and found out he had a hernia. The next day he filled out a company incident report. No days were lost and no restricted duty until he had surgery on 12-27 04.
EE WAS RIDING IN PERSONNEL CARRIER. GOING THROUGH A VENTILATION CURTAIN, EE'S KNEE PAD GOT CAUGHT ON THE CURTAIN. THIS CAUSED HIS FOOT TO GO UP INTO THE TOP (MINE ROOF).
INJURED WAS SITTING ON THE CATWALK AT THE HEAD END OF THE #54 REFUSE BELT CUTTING BOLTS OUT OF THE SNUB ROLLER. HE THEN STARTED TO STAND UP, TWISTED HIS ANKLE, BREAKING THE SMALL BONE IN HIS RIGHT LEG.
CREW WAS TAKING FLAT CAR TO THE END OF TRACK TO TIE IT OFF AND UNHOOK FROM THE MANTRIP. THE HIGHLINE SLED WAS LOADED ON THE FLAT CAR, AS THE FLAT CAR WAS MOVING, THE HIGHLINE SLED HIT A RIB BOARD ON THE LEFT SIDE OF THE ENTRY. THE SLED SLID SIDEWAYS AND PINNED EMPLOYEE BETWEEN BELTLINE AND SLED.
EE was struck by a piece of rock that had fallen between two rows of roof bolts as he was walking back to the roof bolter.
EMPLOYEE WAS TRYING TO FREE UP TWO SECTIONS OF DRILL STEELS THAT WERE HUNG TOGETHER. HE WAS MOVING THE DRILL BOX IN AND OUT USING THE BOOM EXTENSION LEVER. HE MISSED THE EXTENSION LEVER AND HIT THE BOOM SWING. HIS HAND WAS ON THE DRILL STEEL AND WHEN HE HIT THE WRONG LEVER IT SWUNG THE CANOPY INTO HIS ARM, PINCING IT BETWEEN THE CANOPY AND DRILL STEEL.

The employee was decending the ladder of a haul truck when he slipped and fell to the ground. He fell onto his left hip. Upon medical evaluation, it was determined that he had a fracture in the left pelvis region. This accident has reasulted in lost time.
Employee was swinging 20 lb hammer in panel sb14 driving pin enter support frame. felt pop in his left shoulder. ee did not miss any work but was restricted on overtime.
During dismount from a dozer, ee lost his balance and fell, landing on the ground flat footed.
EE WAS IN THE PROCESS OF DISLODGING A PIECE OF IRON FROM JAW WHEN HE SLIPPED.
EE has had previous back problems and is currently off work (effective 11/5/04) and is preparing for back surgery. His physician has related all current problems to the Aug 2002 incident.
Employee was cleaning out tractor bucket with a shovel. When the employee swung the shovel at the build up with a left motion the employee lost his balance. Falling backwards his left hip and left buttocks landed on another removed chunk of clay causing the employee to have a severe buring sensation in that area.
Employee stated that track cleaner came off track. He went back to locate a tie and when walking back with it, he lost his balance causing him to start to fall. When starting to fall, he grabbed for a coal car and in doing so he strained his right shoulder. Due to this employee's work schedule this accident was not determined to be lost time until 11-9-04.
Employee was changing leg jack at the 55 shield, he pulled the come-a-long handle into chest (confined space) bruising the right ribs. later xrays showed Fx to 4 ribs.
WAS HELPING BUILD A BRATTICE WHEN HE PICKED UP A BLOCK & WAS LIFTING IT OVER HIS HEAD & TURNING AT THE SAME TIME. HE FELT PAIN IN HIS LOWER BACK.
EE was pulling conveyer belt off a mine car using channel locks to grip belt. The channel locks slipped, ee went backwards, tripped over 6 inch water line and fell against coal rib.

EE STATED WAS HANGING VENTILATION TUBING WHILE BEING POSITION IN LOADER BUCKET. TUBING FELL KNOCKING EE ONTO FOOTWALL.
BACKING BRIDGES FROM #2 HEADING DOWN #3 HEADING, BRIDGE WAS PUSHED INTO RIGHT RIB OF #3 HEADING.
EE WAS TAKING DOWN BELT STRUCTURE. EE LIFTED A CRADLE AND FELT PAIN IN LOWER BACK AND GROIN, CAUSING A SPASM
THE EE BENT OVER TO PICK UP SOME CHAINS - FELT PAIN IN HIS LOWER BACK.
Employee lifted socket on the end of a steel cable and felt pain in groin area.
EE WAS STEPPING OFF LAST RUNG OF LADDER, STEPPED ON GROUND AND TWISTED KNEE, FELT PAIN IN KNEE IMMEDIATELY.
Employee's lower back started hurting, he had an injury to his back in July. Employee was driving a forklift when back started hurting, may have rehurt lower back again.

WHILE WORKING IN TAILGATE ENTRY, EMPLOYEE WAS IN THE PROCESS OF CUTTING DRISCO PIPE WHEN A SECTION OF COAL RIB FELL STRIKING HIM ON THE HARD HAT.
Employee was cleaning fines out of a screen hopper with a pipe. Some fines came down and the pipe struck him on the side of the head. Employee continued working until 11-10-04 when he requested to see a Dr. An MRI was performed on 11-11-04 to rule out a concussion. The MRI was considered normal and the employee was released to return to work full duty on 11-12-04.
Employee has had a history of spinal sterosis w/degenerative disc disease of his neck & underwent a multi-level cervical fusion in 1999. EE has had ongoing problems with pain & numbness, both arms. EE started taking sick leave on 11/04/04. On 12/20/04 we were notified by the state the EE alleges time off was from an incident which occured on 10/04/04 from pulling on a wrench.
Employee was unloading scrap metal out of a 10 wheel truck. The metal stuck to tailgate as it was being unloaded. Employee lowered the truck's bed and went to loosen the tailgate to release the stuck material. The tailgate released and pinned the employee's left arm between the tailgate and bed of the truck, breaking his left arm.
Employee was operating a roof bolter when he tore a ligament and lacerated his left middle finger.
While operating this cat d9h over rought terrain, the operator experienced a pain in his back that intensified over the next several days.
EMPLOYEE WAS CLEANING #59 SCREW, TRYING TO UNPLUG IT ON THE SOUTH SIDE. HE WAS TAKING SCREW COVER BOLTS OFF AND HOT MATERIAL GUSHED OUT OF THE SEAL, FALLING ON TOP OF HIM. HE HAD PROPER GEAR ON BUT DID NOT HAVE HIS GLOVES INSIDE HIS JACKET SLEEVE. HOT MATERIAL GOT INTO HIS RIGHT GLOVE CAUSING SECOND AND THIRD DEGREE BURNS TO HIS RIGHT HAND AND WRIST.
Crane operater was outside his crane attempting to secure a chain hook to saw track when he felt pain in his lower back. EE stated he was pulling and twisting at the same time. Initial doctor examination indicated a strained lumbar. EE out 10-14 days.
Employee was inserting a roof bolt into the mine roof using the roof bolter to push the bolt into the hole. He was using his left hand to guide the bolt into the hole and got his middle finger pinched between the mine roof and the bolting machine.
EMPLOYEE WAS MOUNTING A 785 HAULER AND FELT SOMETHING PULL IN HIS LEFT ARM.
EMPLOYEE WAS HANGING A LINE CURTAIN IN #4 ENTRY ON #6 UNIT WHEN A PIECE OF ROCK FELL AND STRUCK HIM ON HIS RIGHT LEG. ON 11/08/04 EMPLOYEE HAD A MRI DONE. THE MRI SHOWED THE EMPLOYEE NEEDED SURGERY. HE BEGAN MISSING WORK ON 11/22/04.

EE WAS PUTTING HYDRAULIC OIL IN #2 D9L DOZER. HE STEPPED ON TOP OF THE RIGHT PUSH ARM AND TRACK. EE SLIPPED AND FELL, PULLING AND TWISTING.
Employee was entering a screen deck and felt back pain.
EMPLOYEE WAS STANDING ON TOP OF A CONTINUOUS MINING MACHINE RESTOCKING THE CABLE. HIS FEET BECAME ENTANGLED IN THE CABLE AND HE LOST HIS BALANCE AND FELL TO THE MINE FLOOR INJURING HIS LOW BACK. HE CONTACTED THE MINE 11-9-04 AND REPORTED THE ACCIDENT TO THE MINE SAFETY DEPARTMENT. HE STARTED MISSING SCHEDULED WORK DAYS ON 11-9.
While helping clean out hose, he placed his hand in a pinch point. Operator raised boom pinching finger.
EE WAS SWEEPING THE SHOP FLOOR AND ATTEMPTED TO SLIDE TWO METAL SAW HORSES WITH A CUTTING EDGE ON TOP WHEN THE SAW HORSES TURNED OVER, DROPPING THE CUTTING EDGE ON HIS FOOT.
Employee came to work with Dr's excuse & a prescription for Cecebrex & Vikaden. Foreman told employee he could not operate equipment while on Vikaden. Employee stated that he went to Dr. for pain in his shoulder, wrist and right foot. He also stated tha his right foot was still giving him problems. The Dr. wrote to not use right foot on pedal that Miner Operator has to use.
After using electric drill to make holes to install Flexco connector while repairing underscreen conveyor belt, employee stood up and felt severe sharp pain in low back.
EE was working to replace the sides on a caterpillar 980G loader bucket when the new piece they were fitting in place fell and landed on his right foot.

Employee was operating a terex scraper at a reclemation site and started down a hill when the rear of the scraper slid sideways causing the front to drop down causing the ee to strike his left knee on the dash.

Employee was cutting end of discharge pipe off to install dresser sleeve. He was sitting on top of discharge pipe cutting end off. He stepped on top of handrail pipe; his foot slipped off onto cable. The cable connector broke and he fell on top of handrail pipe and hudraulic pipe injuring his ribs (left side) and his right knee.
Employee was working on conveyer belt; he pushed up on the belt and the belt sucked his hands up over the beam. The belt slid across the top of his arms and he was stuck there. Another ee lifted the belt up and he crawled out.

AFTER BEING LOADED WITH SAND, DRIVER WAS CLEANING THE TAILGATE WHEN THE TAILGATE FELL CATCHING HIS RIGHT INDEX FINGER BETWEEN THE GATE AND THE TRUCK BODY.
Injured was dismounting a rail mantrip when he felt his knee fail. There were no tripping hazards or loose material involved in the incident, only body motion and possibly poor body positioning.
EMPLOYEE STATED THE COAL CARS HAD COME OFF TRACK AROUND 3-SOUTH. AT THIS TIME AN AIR HOSE HAD BLOWN ON ONE OF THE 50 TON MOTORS THAT CAUSED IT TO BE TIED DOWN WITH A MANUAL BRAKE. AFTER ANOTHER MOTOR ARRIVED, THE EE WAS TRYING TO LOOSEN THE BRAKE THAT WAS TIGHT. HE WAS USING A JACK BAR TO TRY TO LOOSEN THE WHEEL WHEN THE BAR SLIPPED CAUSING HIS RT.4TH FINGER TO BE PINCHED.
Employee was cleaning vent on air conditioner and fingers came in contact with fan.
EMPLOYEE WAS WORKING AT THE LONGWALL FACE WHEN HE STEPPED INTO A HOLE. LOST TIME INJURY AS OF DECEMBER 6, 2004.
THE INJURED EE WAS WORKING ON A SPEED REDUCER AT #14 BELT DRIVE. WHILE TURNING THE REDUCER EE GOT HIS HAND CAUGHT IN THE GEARS, RESULTING IN A LACERATION ON THE TOP OF HIS RIGHT HAND, APPROX. 3"" LONG.

ON 11/11/04 EE WAS WORKING ON PUMP AT THE #5 BRATTICE & STEPPED IN A HOLE & TWISTED HIS RIGHT KNEE. HE WORKED HIS REGULAR JOB UNTIL 2/3/05 WHEN HE HAD ARTHROSCOPIC KNEE SURGERY & BEGAN LOSING TIME.
While assembling the crusher components, EE was hitting hte head lock nut with a heavy metal ram and twisted his right wrist.
Employee was backing dozer over fill material when he backed up and over a rock. When the dozer dropped off of the rock it hit hard causing pain to lower back and tailbone. Employee finished shift and continued to work until 11/06/2004. After a trip to an emergency room he was told his tailbone was fractured. At present time employee is still working.

DRIVER STATED HE RAN OVER A ROCK WITH A 980G END LOADER & HURT HIS BACK. HE DID NOT REPORT THIS INCIDENT UNTIL 11/05/04.
EE WAS OPERATING HYDRO SEEDER. HE STATED THAT HE WAS CLIMBING GRADE WHEN MACHINE POWERED OUT. EE PLACED THE MACHINE IN NEUTRAL AND IT STARTED ROLLING BACKWARDS. EE PANICKED AND JUMPED OUT. THE MACHINE ROLLED APPROX. 40 FT. AND STOPPED. EE HAS A SLIGHT HAIRLINE FRACTURE OF HIS TIBIA.
THE EMPLOYEE WAS PICKING UP ROCKS AND PUTTING THEM ON THE BELT WHEN SHE FELT PAIN IN HER LOWER BACK.
USING A PRESS IN THE SHOP TO INSTALL A NEW COIL SPRING INTO A BRAKE CAN, THE SPRING RELEASED AND STRUCK EMPLOYEE IN THE FACE.
EE STATES HE WAS HELPING TO LOAD PANLINE ON A SCOOP. AS HE STEPPED OFF THE PAN TO TALK TO THE SCOOP OPERATOR, HE STEPPED ON A PIECE OF COAL AND TWISTED HIS ANKLE. EE MISSED 3 SHIFTS AS A RESULT OF HIS SPRAINED ANKLE.
Employee was clearing a conveyor, which had been shutdown. A running conveyor caught a piece of loose clothing and ripped it from his body. EE suffered abrasions to his back and left flank area, when his shirts and coat were torn off.

While sliding a liner bar off of a pallet onto a forklift, the liner fell and it hit his foot.
Injured strained muscles in back while pushing bolt up through resin into bolt hole
WHILE MINING X-CUT, 2 TO 1, A ROCK APPROX. 2' WIDE X 36"" LONG X 4"" THICK FELL OUT FROM BETWEEN BOLTS, STRIKING THE INJURED EMPLOYEE BETWEEN HIS SHOULDERS AND KNOCKING HIM DOWN ON HIS LEFT SIDE. THE ROCK THAT FELL OUT WAS 3 ROWS OF BOLTS BACK.
Celite feels this is not a valid injury, employee not working, Supervisor stated injury occurred at home. Claim denied by L&I. New claim using injury date 11-30-04 accepted 1-28-05 by L&I. Acc rpt filled out 1-18-05 stated he pulled pallet from stack set on machinery for supersack bags felt pulling back of leg, pulled another pallet from stack felt stinging lower back.

UNLOADING DRIVE CHAIN FOR HEAD DRIVE OFF OF FLAT CAR CHAIN TWISTED WHEN IT HIT BOTTOM OVER ON LEFT FOOT
Employee stated that he and a helper were lifting a piece of steel metal. As he lifted the metal and turned to carry it, he felt pain in his lower back area.
Getting into a drill truck at mine site, he slipped on entry step, caught himself, but sprained or strained low back.
EE was opening hatches on bulk trailer when he felt burning in right knee. EE was determined to have a torn meniscus on 12-21-04. Surgery performed on 1-5-05.
EE WAS BEGINNIGN TO TROUBLESHOOT A REPORTED STEERING PROBLEM WITH TRUCK, EE WAS SITTING IN SEATM RESTING LEFT ARM ON STEERING WHEEL WHILE REMOVING SHROUD SCREWS UNDER CONSOLE, WHEEL INDEPENDENTLY TURNED THROUGH2-3 REVS, LEFT ARM BECAME CAUGHT IN WHEEL AND WEDGED AGAINST RETARDER LEVER, ARM BROKE BETWEEN ELBOW AND SHOULDER.
Employee was using a pry bar to close railcar hopper doors at GRT#1. The pry bar slipped causing the employee to fall landing on his left leg, resulting in a fractured left ankle.
Employee was checking tension on a pull rope with his open palm when the rope snapped.
Employee was jackleg bolting in 69670 +225 E. While installing the last row of split set bolt a rock 12"" x 5"" x 3"" fell from the back at 11 feet. His left hand was on the throttle of the machine and the rock struck his hand. His left three fingers were fractured. The rock weighed 6 lbs.

ON 12/03/04, EE EXPERIENCED SHORTNESS OF BREATH AFTER FIXING ROLLER. WAS SENT TO HOSPITAL TO REMOVE FLUID FROM RIGHT LUNG, SPENT 3 NIGHTS AT THE HOSPITAL AND RELEASED 12/06/04
EMPLOYEE AND CO-WORKER WERE ATTEMPTING TO UNCLOG A RAYMOND MILL BY HITTING THE PIPE. A BRACKET, UNATTACHED TO SILO BUT CONNECTED TO PIPE, DISLODGED AND FELL APPROXIMATELY 75' STRIKING THE EMPLOYEE.
WHILE EXITING THE LOADER CAB, EE SLIPPED AND GRABBED (LEFT HAND) PART OF THE LOADER TO PREVENT HIS FALL. EE CONTINUED TO WORK AND FINISHED HIS SHIFT. HE THEN WENT TO THE LOCAL DOCTOR. WAS SCHEDULED TO SEE A SPECIALIST ON 11/30/04: DIAGNOSED WITH STRAINED MUSCLE IN LEFT SHOULDER. HE IS SCHEDULED FOR 3 WEEKS, 3-TIMES-PER-WEEK, OF PHYSICAL THERAPY.
INJURED STATED THAT AS HE BENT OVER TO MOVE THE BOLTER CABLE HE FELT PAIN IN LOWER BACK AND LEFT LEG. INJURED WAS TAKEN TO UNION CO HOSPITAL.
Employee was putting some pipe on a screw conveyer and strained his back. He told his supervisor but did not want to file a report or seek medical attention at that time. First Dr visit was 1/21/05- restricted duty.
EMPLOYEE WAS LIFTING A BOLT SUPPLY TRAILER TOUNGE WHEN HE FELT PAIN IN HIS BACK BETWEEN HIS SHOULDER BLADES & THEN IN HIS RIGHT ARM.
Injured was installing crane boom extension (jib). EE was standing on a ladder to place jib pins. Jib was being hoisted into place with an RT fork truck. EE placed himself between boom and jib and was caught between the two pieces, resulting in 2 fractured ribs.
Fire extingisher mount came loose, knocking out safety pin allowing extingisher to release contents. Operator couldn't get out of loader fast enough and inhaled some of the contents.
CLIMBING TRUCK LADDER AND BUMPED RIGHT KNEE ON TOP STEP.
Driller's helper (Progressive Diamond Drilling) was removing core from a core barrel when he was struck by the core and sustained an avulsion to his right middle finger pad. Medical exam shows no fracture. The driller received 4 sutures.
EE WAS CHANGING BITS ON THE CONT MINER, SLIPPED AND FELL AGAINST THE SIDE OF THE GATHERING PAN ON THE MINER.
Employee was driving a cat 771 haul truck and felt pain in upper back the following day.
While lifting a suspension bar to install in the hammer mill, ee felt something pull, had gone to Healthworks 11-17-04, returned 12-02 to Dr, determined surgery was needed. Surgery scheduled for 12-8-04.
EMPLOYEE WAS PLACING ROCKS ON ROLLER CONVEYOR. A ROCK FELL BACK ONTO HIS HAND AND BROKE HIS RIGHT RING FINGER.
EMPLOYEE WAS HEATING A BRACKET THAT HOLDS THE DUST BOX ON A 320 CAT EXCAVATOR WITH A DRILL ATTACHMENT. AFTER HEATING THE BRACKET, EE WAS ATTEMPTING TO STRAIGHTEN THE BRACKET USING A LONG PIECE OF PIPE TO APPLY PRESSURE TO THE BRACKET. EE WAS STANDING IN A MUDDY AREA THAT HAD LOOSE MATERIAL. THE PIPE SLIPPED FROM THE BRACKET & EE SLIPPED, FALLING ON HIS LT. LOWER LEG BREAKING IT.
SLIPPED AND FELL IN SLURRY TANK HURTING LEFT SHOULDER/NECK
Employee went to the Shipping break room to take break. He sat on the end of an aluminum bench. Because there was no other weight on the bench, it tipped and flipped up, causing him to fall on the floor. He sustained injury to his lower back.
Employee was driving a Dresser water truck. He hit a pothole and experienced back pain the following day.
Employee was making adjustments to the bed knife and rotor blades on the granulator when his hand was pinched between the two.
Employee was welding on a drag line chain and twisted his back, when he moved the chain. He was examined by three Physicians and was found to have two ruptured disks. Filed on works comp., hurt on duty

Allegedly, while walking into finish mills motor control center, employee tripped over cement build-up twisting his back.
RAIN, WET, LOOSE GRAVEL, WHEN REMOVING ROCKS FROM GRIZZLY. SLIPPED ON GRAVEL & FELL ON GRIZZLY RAILS.
EE WAS INSTALLING A DRIVE BELT ON A SHEAVE. FINGER WAS CAUGHT BETWEEN THE DRIVE BELT AND THE SHEAVE. SHEAVE GROOVES WERE WORN, CAUSING THE EDGES TO BE SHARP. FINGER WAS SEVERELY LACERATED WHEN PRESSED INTO THE GROOVE OF THE SHEAVE.
Employee struck his right elbow on a 3/8"" spray nozzle in the spray bar while installing a steel liner on the feed end of sand screen. X-rays at clinic revealed no positive results. He was placed on restrictions by doctor but was able to perform normal job duties within restrictions. Visit to doctor on 1/10/05 said no work until 2/14/05.
EE WAS TRYING TO REMOVE A SIDE STAB SHOE BRACKET USING A HYDRAULIC PORT-A-POWER. EE FELT A PAIN IN HIS BACK WHEN HE TWISTED TO ACTIVATE THE POWER SWITCH.
EE WAS WALKING THE THE QUARRY, SLIPPED ON SOME ICE AND FELL
EMPLOYEE WAS CLEANING OUT JAW THAT WAS FULL OF ROCK. WHILE BENDING FORWARD AND SHOVELING OUT JAW, ROCK GAVE WAY FROM THE OVERHEAD FEEDER AND FELL ONTO HIS LOWER BACK.
Employee was struck by a shuttle car cable on the left knee. Employee was hanging cable back to the transformer when slack was taking up on the cable reel causing the cable to whip against the employee.
